Found an unintentional roof fall in Break #4 from #3 Heading to #4 Heading in the 1st Left off of Right Shell Submains. Dimensions of fall are approximately 60' long by 20' wide by 6' to 8' thick.
A Roof Fall occurred on the #2 unit face area inby spad 49+60 along the 2nd south submain off the Sturgis Mains in the #3 entry. The fall measured approx. 20' wide, 40' long and 6' plus high.
While foreman and State Inspector were traveling Intake airway they discovered a roof fall in the #4 southwest mains measuring 20 feet by 20 feet by 8 feet thick. In the #2 entry.
#3 Hoist-electrical faults. nt. upper gate, south lower gate open and ground fault.
An unplanned roof fall was encountered in the First Right Panel off of the South West Mains Section of the mine. The fall occurred near survey station 1675 and extended outby a distance approximately fifty foot. The size varied from five to six foot in thickness. All approached were previously dangered and breakered off. The fall has been marked on the mine map as required.
ROOF FALL - Occurred in the #3 entry, sat down on the #2 longwall crusher and fell above the 6 foot roof bolts in the #4 entry. There was an open area in the #3 and #4 entry that allowed positive air flow to the face. No injuries occurred at the time of the roof fall and access to the escape ways were maintained.
Elevator was inoperable at 11:55 PM doors would open but the elevator would not go down. No men were trapped at the time of the incident. Elevator was operable at 12:45, SCR Drive Torque Processing Failure.
A rock fall occurred on2/24/14 in #6 entry at 3+00 in the 2nd North Panel measuring 25ft long x 18ft wide x 8ft thick. The fall has been cribbed around.
while mining in number 4 entry in E-3 butt the continuous mine struck a well casing approx,.6-8 inch hole dia approx 9-10 inch no gases present water coming from pipe approx 6-10 gal a min federal inspector was onsite issued a K order at time
A roof fall was discovered on this date. The fall was located in the #3 entry at crosscut #9 of the mains. The fall measured 50' long by 19' wide and was 9' in thickness. The area was originally supported with grouted rods. The area was dangered off with timbers and cribs. The mine is currently not producing and no employees were affected.
Hoist car was down due to an electrical problem.
Employee entered an elevator near Empire Pellet Plant Repair Bay. Compartment proceeded to rise when elevator stopped between levels. Employee used portable radio to communicate situation. MSHA Hot line called 15 minutes into event. Employee ""freed"" after 50 minutes. No injury. Service company found open fuse in system, but no known reason for condition.
IGNITION - Occurred while cutting in the #3 face on 8 section. The flame was orange in color and burned 10 - 15 seconds and extinguished with water from wash down hose.
The mine operator withdrew miners and equipment from the AII Railroad Pit when heavy rain began to fall. Approximately 1.28 inches of rain fell over a period of thirty minutes. A berm was constructed at the top of the ramp to prevent more water from entering the pit. The Safety Specialist called the local MSHA Supervisor who advised him to call the MSHA hotline.
An unintentional Roof Fall has occurred on the #1 Section 2nd Left plan on Chase SubMains spad 9420. The fall was approximately 60ft x 19ft x 4 1/2ft. There were no injuries reported and no equipment was involved.
Coal and rock roof approximately 33 feet long by 10 feet wide and up to 7 feet high fell on and around the long-wall stage-loader.
JLB Trucking (contract coal hauler) was on site preparing to dump a load of coal. Driver said he heard a pop and then smoke and flames entered the cab of his truck (#22.) Driver used his portable fire extinguisher but it did not extinguish the fire. The Gary Fire Department was contacted to fully extinguish the fire.
Sandy ground where a slab round was loaded too heavy and timed poorly.
A roof fall was reported in the North Mains 2, #2 entry at spad 5205.
While burning bit lugs on the head of a continuous miner a gas ignition occurred on the right rib beside the miner head. The miner was located in the #4 entry on west main section approximately 30 feet out by spad 96+4.
At approximately 11:30pm the slope hoist car became inoperable. The batteries had to be replaced for the braking system and the brake car was back in operation.
At approximately 12:45 pm a large quantity of CH4 gas began blowing from the gob area near the head gate (unknown if it was the roof or floor) and quickly exceeded 1.5% CH4 on the face and soon over 2% CH4 in the bleeders. The panel was evacuated, return air monitored and the CH4 returned to legal levels after about 36 hours. CH4 levels have remained at legal levels since then.
Radio Communication system was down due to an electrical problem.
A fall of roof occurred in the 2nd NE at crosscut 37, # 7 entry. The fall measured approximately 18 feet wide x 18 feet wide x 12 feet thick. The area was supported by 6 foot fully grouted roof bolts.
During the night operations, a hauler operator noticed movement in the spoil near the loading area.He contacted the loader operator over the CB. The supervisor overheard the conversation and removed the hauler operator and the loader operator from the pit. The slough was contained by the safety beam at the toe of the slop with only minimul pit encroachment.
A 13.8KVA breaker failed cutting power to the 8 Shaft hoist which was in the process of hoisting 4 miners out of the mine. The men were stuck in the shaft for 2 hours and 40 minutes, alternate power was used to run the hoist. No one was hurt and other than the breaker no equipment was damaged.
While working on his communication line an electrican identified a roof fall in the #4 entry near spad #2775. The fall was approx. 20'W X 30'L X 3'-4'H the area was mined approx. March 1992. The area was supported with 4' fully grouted resin bolts and will not be cleaned up. All approaches have been dangered off and timbers have been set.
5 men were entrapped on Sugar Run elevator from 6:00am to 7:40am. Ice built up on governor rope and tripped governor switch. Ice came from a small hole in the shaft that allowed a water seep 100' from top of shaft. RG Johnson came down and put a plug in the hole to stop the leak. Ice removed from rope and shaft wall.
Cleaning service person used office elevator to access another floor (2 story bldg.) when the elevator got stuck. Total time of entrapment was 38 minutes.
5 men were entrapped on the sugar run elevator from 6:05 am to 8:35 am. Ice built up on the governor rope and tripped the governor switch. Ice removed from rope and wall beside governor and hung brattice by opening that was blowing snow onto the rope. Elevator returned to service at 1:15 pm.
The emergency hoist would not travel to the bottom of the shaft within the 30 minute time frame, due to ice on the rails in the shaft.
The Crabapple Elevator was not operational for an extended period of time. The frequency drive was found to be at fault and was replaced.
An unintentional roof fall occurred at or near spad #2237, 25 cross cut, #8 entry, 2nd NW Mains. The fall was 55'X20'10-14' high, 2300' outby the working section. It appears to be a slicken side rock formation. No rider seam is present
Multiple hill seams in #5 entry along #5 belt in route to 003 inactive section which is 1 of 3 intake air courses.
A roof fall in #3 entry on #3 belt from brk.13 near spad #34 and continued to spad #40 on brk 15. This is a continuation of a previous fall that had been cleaned up and re-supported.
Electrical breaker in control panel tripped on #2 hoist resulting in a short outage. The mine was evacuated and the breaker was repaired. Hoist restored to service and miners returned underground at 12:25 p.m.
A roof fall was discovered at the #7 wall intersection in the track entry of the old 6 East Longwall section. No injuries. The dimensions of the fall are 30 feet long X 15 feet wide X 8 feet high.
The #1 elevator would not leave the top landing. The car cable and the top landing level of the k limit switch were replaced. the car was out of service from 3:30pm and returned to service at 6:45pm.
The #1 elevator would not leave the top floor. the car was out of service at 12:10am. At 12:30am the door lock was found to be the problem. The lock was cleaned and lubes and tested. The elevator returned to service at 1:10am
Employee was entrapped in the #2 portal elevator for 32 minutes. The elevator's governor was adjusted and repaired.
Roof fall at x-cut 138, entry #1 of the Headgate #1 Unit.
While examining the intake, the mine examiner found a cave twenty-five foot outby spad 19893 between four to five entry. The cave measured sixty foot long and six foot high. The cave area was supported with four foot fully grouted bolts. The area was timbered off and abandoned. All work was done according to the approved plan.
A Cemex Laborer and a temporary Labor Finder Laborer were traveling in the elevator and it stopped between the third and the fourth floor. Executive elevator was called to troubleshoot and repair the elevator. The men exited the elevator at approximately 03:20 AM. There were no injuries as a result of this incident.
A roof fall was reported in the old 3rd left B-3 panel, one break inby spad 2047.
A roof fall on the belt line. 2ND South East Submain (3+70) XC#5, Entry # 6, Approx 50' in length, 18.5' in width, greater than 6'in height.
An unintentional fall has occurred on the 2nd west travelway at #50 crosscut due to deterioration of the roof above the bolt anchorage.
An unplanned fall of the mine roof above the anchorage horizon of the roof bolts occurred on the 7 Right section at #3 block in the #2 entry.
An unintentional fall occurred on the main north travelway at #50 crosscut due to deteriorations above the bolt anchorage.
An unintentional roof fall has occurred on Horse Creek Mains #7 Entry Right Return 1 break inby spad 374. The fall was approximately 40ft x 18ft x 6ft to 8ft. No injuries were reported and no equipment was involved.
A roof fall occurred in the #9 coal seam in the panel 11. The roof fall occurred in the primary escape way at crosscut #44 in the #7 entry. No injuries or ventilation short circuit occurred as a result of the roof fall.
A roof fall in the 5th South West Submain (9+70) XC#12 Entry#3. The fall was on a pump and pump cable. Aprox 20' in Length, 20' in width, greater than 6' in height.
Plant power kicked off at 7:15 am on 2-6-14. Plant power was restored at 7:20 am but hoist power remained tripped. electricians worked on problem with main breaker, Called MSHA to report hoist power outage. Main breaker was repaired and hoist was running at 8:17 am. Second call made to MSHA to report problem resolved at 8:27 am on 2-6-14
While traveling with an MSHA inspector on a weekly examination route, a reportable roof fall was found in F Seam, Southwest Main Intakes, entry 3, at crosscut 22. The fall was partially located in inactive works, but affected the established travel route for the weekly examiner. The fall was approximately 6' high, 18' wide, and 38' in length.
The emergency hoist became inoperable due to ice build up in the shaft. The ice was removed at 7:10 AM on 02/06/2014 and the hoist returned to operation at 7:15 AM on 02/06/2014.

An unplanned roof fall above anchorage was discovered at approximately 8:00am, one crosscut inby the #2 Mine Fan installed in the #7 Portal entrance. The fall measured approximately 16 feet wide x 20 feet long x 6 feet high in the intersection of Survey Station 21.
The tip at spad 253 about 8 ft passed spad to fell about 16' x 16' long and about 16' x 16 wide looks like just a horse back in the middle brought bolts with it the horse back was above the bolts. Don't know when this happened. Just found it.
At approx. 5:35 am, a roof fall at or above the anchorage was discovered in the A entry return of the 4North Mains. The fall was approx. 20 ft long 18 ft. wide and 6 ft. high. There were no injuries or property damage. The fall was dangered off and the return was rerouted.
Escape elevator was being operated to task train an employee on its use and the hoist would not run its full course. Crews were immediately evacuated.
Loader operator backed the loader away from the rock pile and hit a granule truck that was behind him. The granule truck was moving as well. There were no injuries and some damage to both vehicles. The Loader operator was taken in for a drug screen and was cleared to go back to work.
An unintentional Roof Fall has occurred on the #1 Section #5 Entry between pillar blocks inby spad 10093. The fall was approximately 90ft x 19ft x 4ft-6ft. There were no injuries reported and no equipment involved.
1 person was entrapped from 12:55 to 2:55 am on Sugar Run elevator due to an electrical fault (governor switch tripped). The switch was reset, miner removed, the contacts were cleaned on the starting panel relay for the motor fields. Returned to service at 3:50 am. No injuries, no issues with company rules, regulations, PPE or training.
While doing weekly examination a rock fall was found on 4 main Break 17 in the #4 entry. The size of the fall was 8' thick 18' wide and 30-35' long. The ventilation is not affected. Fall occurred due to a transition zone in the top. An action plan has been submitted and is still pending.
Elevator operator ran the elevator (not for emergency use) and it would not travel all the way to the bottom. Crews were immediately evacuated.
A LNG truck was attempting to leave the mine without putting chains on his drive wheels. He asked an employee to pull him up the hill with the John Deere loader, they attached a 20' cable between the loader and truck. The loader could not make it up the hill, backing down to unhook the cable, the loader slid down hill on the ice and contacted the right front of the truck.
At approximately 2:35 pm the service hoist went down due to the north gate switch at the bottom of the shaft. Switch was repaired at approximately 3:30 pm and the service hoist was put back in service.
6 people were entrapped on Sugar Run elevator from 12:58 to 3:10 am due to an electrical fault (governor switch tripped). Switch was reset, men were removed and fault was unable to be reproduced that day. Later had to clean contacts on starting panel relay for motor fields. No issues with company rules, regulations, training or PPE.
A miner become buried from material sliding down the face of a bank.
Emp a contract worker repairing a Scaler in Mod 12. The emp had fallen to the ground in fetal position, locked arms, clenched fist and bleeding from the ears. The witness did not know if the emp fell from a height or ground level. The injured emp was airlifted. He was lacking oxygen to brain for 10 minutes. He was pronounced dead at 9:45AM on Feb 28.
Without checking to ensure elevator car was present, contractor stepped into elevator shaft and fell approximately 25 feet landing on top of elevator car.
Employee was tramming continuous miner from #4 heading toward #1 heading when he was caught between the conveyor boom and the rib. There were no eyewitnesses.
Miner became entangled in belt return roller and sustained fatal injuries. Equipment involved was Rahco mobile stacker, Model m30x1500.
Employees were attempting to install a 8""X8""X1"" steel diagonal brace. The upper end of the brace slipped off the connection clip on the support column and struck the right hand of Employee #1 pinching it between the diagonal brace and the inside of the support column. Physicians at UMC-Tucson made the decision to amputate the right pinky finger.
The employee was helping to move a large steel plate, one of the chains slipped and the plate fell on his right foot, partially amputating the right great toe. Surgery was required and the toe was amputated at the first knuckle. Employee placed off work. This will be 150 lost days for the amputation at the first knuckle of the great toe.
Injured employee was working in the #9 heading of 1 North Mains when a piece of rock fell from a rib and struck his left foot resulting in a fracture.
Employee was removing lids from the primary screen when he felt a burning sensation around his belly button. The employee was diagnosed with a hernia on 2/17/14 making this a recordable, although the employee has decided to return to work without any restrictions or having it repaired at this time.
Employee was replacing cat pads on the continuous miner. As he positioned the cats for installation, his finger was caught between the joint of the pads. This resulted in the amputation of the distal portion of the left little finger.
The haul truck operator's hand was grabbing onto the hydraulic tank ladder at the hinge point when the tech stepped on the bottom rung causing the ladder to settle against the stops. This movement at the pinch point amputated the tip of the tech's right index finger.
Employee pinched his finger in the snatch block causing the tip of his left hand pinky finger to be removed.
Rough terrain and speed while moving material employee hit bump while running skidloader and jarred shoulder and neck
EE was riding a belt ride up a hill break 16 when he said that it quit pulling and went backwards. That when his passenger jumped off ride another ee said he cut it into the rib to keep from hitting him that when it knocked him off of ride. He said he thinks he hit on his SCSR and hurt his back.
Employee was getting out of forklift when his foot got hung and as he was leaning forward to catch himself the incident occurred. Broke bone in his left wrist.
Employee was leaning into mantrip giving out ear plugs unaware of his right foot was half way under mantrip, mantrip moved catching right foot under mantrip and twisting foot. Employee received a fracture to his right ankle. Employee has returned to work with a full release.
While shoveling belt on his knees Employee twisted to put coal on belt and felt a sharp pain in his back. He was treated Prime Care 12 in Princeton where xrays were taken.
EE conducting a pre-inspection walk around when a chunk of mud fell on top of his head, went to hospital, and has to go to physical therapy.
EE was in the process of loading 6' glue from a scoop bucket onto his bolter. He had the bundle of glue on his shoulder and twisted around to place glue onto the bolter, He felt a sharp pain in his lower left abdomen. Shortly after a knot the size of an egg popped up. He was transported to the hospital where he was diagnosed with a hernia
At 2 block in the 60 pound at Sugar Run the employee dropped to his knees so that he could install a bolt in a low area. He felt a pop and pain in his left knee when his knee struck the floor. No issues with regulatory compliance, training, or mining equipment. Employee was wearing knee pads.
EE was on his knees picking up TC plaster into motor. EE felt pain in the center and lower part of his back and neck.
Employee was pulling power cable to put on the tub at the Dragline. His right foot slipped twisting his lower back. On 2/28/2014 employee went to Logan Regional Medical Center and was examined by Dr. and diagnosed with a Lubosacral Strain and taken off work and referred to a specialist.
He is a pipe man who was carrying a water pump through a crosscut toward the beltline. He was wearing all of his protective equipment and his boots were in good condition. He slipped and tried to hold onto the pump. He felt a pull in his right shoulder. On 3/6/14 an MRI revealed a torn rotator cuff.
Employee was standing on a bucket while retrieving leaky feeder cable and fell onto side of body injuring his neck.
Injured employee filed accident report on 3/20/2014 stating that he had been struck in the parking lot by another employees personal vehicle after work. Injured sustained a broken wrist from the incident.
While inspecting welds of inlet plenum section of Main Bag house, employee stepped in hole for inlet duct. Employee fell approximately three (3) feet onto inlet vane.
Employee was hanging a miner cable in No. 3 entry when he though he pulled something in his stomach area. He continued to work until March 20, 2014 when he starting missing work. The incident was under investigation due to a similar injury previously. On April 2, 2014 the investigation concluded that this was a new reportable incident.
On 2/26/14, employee filled out an incident report for straining his right shoulder. He continued to work until 7/30/14 at which time he had surgery to correct his shoulder problem, after receiving confirmation that his claim was accepted as a work related injury. The difference between the injury date, surgery date and reporting date was pending acceptance of this claim.
Employee stated that while loading a communication and tracking node, he strained his back. Employee went to the dr on 2-28-14 and was placed off of work until 3-3-14. He did not report the incident to the mine site and turned in vacation days for his days lost. The mine site was notified by workers compensation on 3-10-14 that an accident had occurred.
Employee exited the tractor cab on the passengers side facing outward away from the truck and was dismounting, when he missed the last step causing him to fall onto the concrete floor, straining his groin. He was seen at the ER and excused from work.
Employee stated that he was walking in the number 7 entry when he stepped on a piece of coal and twisted his left knee. This resulted in a knee sprain.
Employee states that while working on the 5b head,(002 Section belt head), he stepped off the tail piece and lost his footing causing his (L) knee to twist.
Employee injured his shoulder on 2/26/14. While operating a piece of equipment he ran over a cement spill that had dried. This caused him to strike the cab injuring his shoulder. Employee continued to work up until 12/30/14 where he went off for surgery to repair the shoulder. Employee subsequently retired on 12/31/14.
Employee was attempting to light burner pipe. When he put the torch in the flame went positive causing him to get second degree burns to his face.
Employee was using a punch lock banding tool to splice waterline and while tightening band the band broke causing his momentum to shift forward and the banding tool struck him in the mouth. Employee received 4 sutures on inside of lower lip and lost 2 teeth.
he was upstairs blowing out JM3 filter, coming back down the stairs and slipped in the oil on the steps and to keep from falling he leaped to lower platform and injured lower back.
The employee was walking down the stairs of the #1 Loadout, when his right foot got caught on the steps. The employee's doctor informed the employee on April 28, 2014 that he is scheduled for surgery on May 7, 2014.
Employee was plugging up a new set of scoop battery jumpers when it arced and flashed causing his eyes to burn. Employee was treated at Raleigh General Hospital and was treated with eye drops
Injured was exiting the water truck and slipped from the top step because of material built up on steps. Injured slipped approximately 4 feet and twisted his left ankle when he landed.
Employee slipped and lost his balance falling backwards and landing on his self rescuer as well as a bit bucket. The injured stated the bit bucket struck his lower back. Injured had complaints of back/tailbone pain and tingling down his right leg.
Employee was shoveling gravel from a flat car, and felt pain in his right shoulder.
EE was assisting in change out of skid steer bucket. EE was having difficulty getting the bucket locking pins to release. EE was standing in front of skid steer bucket when locking pin finally released. When the bucket came free from the machine it landed on the employees left foot resulting in dislocation and multiple fractures.
Employee felt a strain in his lower back while handling hi-tech pump able bags.
Turned ankle while stepping down. Refused to go to ER. The incident occurred under previous ownership Arch Coal. Pine Branch took over the properties on March 4th. On April 16th a citation was issued to Pine Branch for failing to report. This report is being completed to avoid further action being taken against Pine Branch after discussions with MSHA.
Employee slipped on ice while exiting his pickup truck in parking area of Refuse Site. He caught himself on his running board with his elbows and injured his left shoulder. Employee continued to work until 6/10/14 when he received arthroscopy surgery on his shoulder.
Employee was walking through door at C2 and struck his head on the top of the door jamming his neck. Employee finished his shift and did not seek medical attention. Continued working until 9-23-14 when he was taken off work following an MRI - diagnosed bulging disc with nerve damage.
He was helping to move the rail bender from one rail to the next when moved the bender down to clamp it in place his finger was between the rail and bender causing the accident.
Going up wet stairs foot slipped. Fell down and twisted back. Stairs have clay and wet came off of peoples shoes.
Employee was changing a shut-off valve on a 4"" inch water line, the straws that hold the coupling in place were broke so he used a hammer to bust the coupling which was still under pressure and it came apart striking him in the left side of the face causing cuts which required sutures and bruising and a broken orbital bone above the left eye.
Employee states while rock dusting struck head on mine roof!
Employee was reaching over pellet mixer and bumped bottom rib on left side.
Employees were using a scoop to move a roof bolter when one of the employees accidentally got his left arm between the scoop and the bolter resulting in a fracture to both bones in his left forearm.
Cutting a lifting waterline (8"") and strained his back.
Employee climbed onto the work table to move a stone. While pulling on the stone, a piece broke off causing employee to lose their balance and tumble backwards off the table striking their head.
The employee was dragging a piece of curtain and stepped in a hole causing a fracture to his right lower leg.
Walking down a belt line and stepped in a hole and fell and twisted his hip and lower back. This was on 2/24/2014 it was a reported only accident. On 3/5/2014 he went to see a Dr. an x-ray was completed and the results were negative. He was still having some pain so a MRI was completed on 3/25/2014 that revealed a fracture to his hip. Placing him off of work.
Employee was installing 12"" PVC drain pipe on top of coal silo. As he was raising the pipe with a chain hoist, the pipe slipped and struck him on the back on the neck/shoulder area. Note that employee did not report this until the following day.
Employee slipped on ice resulting in a contusion to the back.
Employee was in the process of climbing up a ladder, when he lost his balance, and fell approximately 6 to 8 feet to the floor level. Employee lost his footing and center of gravity, when climbing up the ladder, possibly due to sharp knee pain experienced, prior to the fall.
Employee states while hanging roof bolter cable felt pain in middle center of back.
Employee states he was dusting ribs by hand when he slipped and fell and dislocated his right shoulder.
Diverter going to two Quality Control (QC) bins was sticking allowing leaking of product. Work order was submitted at 8:00 am to troubleshoot leakage and repair.
Injured employee stated that he tripped over a section of rock dust hose and felt pain in his lower back.
Breaking bolts loose with wrench in an awkward position wrench slipped something popped in left wrist - acute sprain
Employee slipped on ice and injured neck.
Employee was hand scaling. The company assumes that when the loose was barred down, it struck an electric cable hanging underneath it. Striking the cable caused the loose to spin and struck the employee.
Employee was unloading timbers from a scoop bucket and strained himself in the groin area. Started missing work on 3/5/2014 due to swelling and complications that will require surgery.
EE stood up after eating lunch, felt dizzy and fainted. When he fell, he hit his shoulder on floor. He had been under a doctor's care for bronchitis and was taking medication to lower his heart rate which was measured at 45 beats/minute.
EE was working on the miner with the mechanics and picked up a flex side for the miner off of the Diesel ride, he then walked about 8ft and put in the ram car bed and then got in the car and took it to the miner. He then felt pain in his lower back on the right side. It was about an hour after picking the flex side up that he felt the pain.
EE was getting ready to set over to finish cleaning up 9 right, when a rock fell out between the rib pin and the rib striking him on his left shoulder and left lower leg resulting in fractures to the tibula and fibula in his left leg.
Employee was running the rock dust tanks when the fill valve opened unexpectedly causing a release of pressure causing the employee to fall resulting in a fracture to his left thumb.
Employee swung boom of roof bolter and caught his right index finger between the roof bolter canopy and the coal rib.
992D loader loading rock trucks in pit area when loader wheel went in soft spot in pit causing bucket to hit bed on rock truck jerking operator - causing sore back.
EE was operating a walk-through truss bolter on #3 unit. He was standing on the catwalk of the bolter. He had right arm resting on the tram lever guard when a piece of rock, 14"" x 14"" x 2"", fell and struck him on his right forearm.
Employee was lifting a cover to put back on continuous miner. While lifting the cover the employee experienced back pain. Employee continued to work, but pain did not get any better, so employee started missing work on 03/24/2014. After a thorough investigation by management, it was deemed a lost time accident on 04/08/2014.
EE was blowing off the lime that was on top of an environmental tarp of a loaded truck. When he started blowing the lime off the wind started blowing and blew the lime back into his face and the lime came in behind his safety glasses and got into his eye.
Driver tripped and fell over fuel piping causing injury to right shoulder. The fuel delivery driver was setting up to deliver fuel into tanks located at bulk storage fuel farm. Tanks are under shelter and enclosed. It was dark outside and inside of shelter is dimly lit. While gathering tank reading to determine ullage, the driver lost his where a bouts and tripped over piping.
Employee was loading a KPL motor when the D ring broke and struck the employee in the upper right thigh. PPE was not a factor, mining equipment was not a factor, mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.
He was filling in on section as a utility man. He was standing next to the continuous miner while the operator was cutting coal. A piece of material 4 feet long by 2 feet wide and 1 1/2 inches thick came out of the top and struck his left shoulder causing a separated shoulder. He was wearing all of his Personal Protective Equipment and the crew stated that the top was good.
Slipped on wet concrete ramp and twisted his knee not serious however, this will be lost time accident.
Employee stated that he was changing a bottom roller on the stacker belt and as he lifted up on the roller he felt pain in his right side. This resulted in a chest strain. Employee began missing work on 2/24/14.
While employee was advancing shields, reached down to pick up rock dust bag and the employee right leg came into contact with shield and pan line. Went on restricted duty on 3-26-14
He is an inexperienced miner with 3 weeks experience. He was loading debris onto a mine flat car. A piece of old belt structure fell over and landed on top of his left hand fracturing his left ring finger. He was wearing metacarpal gloves.
Prying 85 lb. track rails into center of track with pry bar, wearing proper safety equipment, trained on job task, felt pain in center of lower back, individual continued perform normal job task until 4/02/2014 when doctors removed from work.
Injured employee stated that he was carrying cables and power supply boxes from A Shaft to the -1 Spur when he felt pain in his lower back.
Employee was tramming a golf cart towards #3 entry, while tramming the golf cart the employee trammed over a ledge causing the golf cart to overturn.
While employee was checking his bolter head for suction, his middle finger on his right hand became injured when it contacted the broken chuck.

Employee had crawled under belt line to cut piece of belt structure. After he finished, he attempted to turn from laying on his stomach to his back. He heard something ""snap"" in his chest as he tried to turn over.
EE hammered a pin and a sliver of metal hit his right eye. He went home, his wife took him to a clinic. Dr. removed the sliver. He had two follow up appt. with Dr., Dr. made ee an appt. with another Dr. (Cornea specialist) 3/12/2014 in L.A.
Employee was operating a Continuous Miner and while standing stationary he had to turn and twist his body to see the approaching Ram Cars. He felt pain in his right knee. He had right knee arthroscopy and partial medial meniscectomy on May 13, 2014.
Employee was installing a mounting pin in a hydraulic cylinder on an excavator and strained his shoulder.
EE was welding angle braces to the load out conveyor truss due to structural deficiencies. He was working from a temporary platform. EE was wearing a harness & had 2 lanyards attached to the structure. For an unknown reason, he unharnessed the lanyard from his harness & slipped from the platform & fell approx. 25' to the ground below fracturing his leg, pelvis & a vertebrae.
Employee was operating off side right roof bolter. He was putting up roof bolt and drill pot kicked sideways and bent drill steel. The bent steel struck employee above right temple.
The employee was inspecting the area of the air blasters which are located in the Cooling System (Cooler), employee placed his right hand against a fan/blower setting it on a plate to revise the blaster, when employee removed his hand from the fan the plate fell on his hand pushing his against one of the air blasters and then pinching his ring finger of his right hand.
Stepped out of bulk truck slipped and fell on ice twisting left knee. Sought medical attention on 2/28/14 full release. Seen specialist on 3/13/14 full release. Still having pain had MRI 5/23/14 was put off for surgery on 6/2/14 with a torn meniscus.
The ee was in the process of tramming miner and the machine became fouled. The ee de-energized the miner and was traveling on the right side of the machine when a piece of rock measuring 12"" X 11.5"" X 5' rolled out and pinned his right leg between the rock and mine floor resulting in two breaks to the tibia.
Employee was using a bar to lift up on a track tie when he was struck by another employee swinging a track hammer. He was struck on his left index finger causing a fracture. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.

Employee stepped off the tailpiece and twisted his left knee. Employee later required surgery and started missing 05-15-14.
Employee stated that her pain started hurting on 2-12-14 when she was operating the forklift. On 2-17-14 while taking parts to yard in the forklift her tail bone & back continuously hurt and was uncomfortable to sit.
Employee claims he was roof bolting & installing monkey face plates in the 402 beltline. As he attempted to install a bolt it bent & he removed the bolt from the wrench & he re-attached the wrench to try to straighten the bolt. As he was doing this the plate hung up & then released & swung around striking him in the back of his right hand.
Employee's duties include sawing, sorting, and packaging stone. According to her report, she lifted a stone from the conveyor and while walking carrying the stone she heard a ""pop"" in her knee. Immediate medical attention was refused and employee remained at work for remainder of her shift, but she visited the emergency room for treatment the evening of the reported injury.
EE was operating a continuous miner. He bent over to move a loop of cable that was in the travelway and felt a pain in his lower back.

Driver was attempting to drive his truck down a hill, and although it had just been graded the truck lost its traction and starting turning until it turned over.
A portion of a coal rib was being removed with a jack hammer for the purpose of correcting an issued close clearance citation. The injured employee was removing the loose coal away from the rib when a rock brow fell onto him.
Employee states being towed through the mud at mine site (after getting stuck in mud) created jerking motions which resulted in shoulder, rib, neck, and back pain. Pain could also possibly be due to repetitive motion involved in assigned duties of driving. Driver first reported to Employer on 2/17/14.

Thursday Feb. 13, 2014 at approximately 10AM an EE and IE were replacing drive belt on 16X20 shaker Screen. IE turned & felt a sharp pain go through his shoulder, the other EE asked if he was ok, IE said yes lets just get it done. Over the course of the weekend the pain didn't ease up. IE went to Dr. Waiting on Dr. report & schedule MRI.
EE(equipment operator) was assisting another ee(mechanic) on equipment maintenance. A frozen dirt clod fell on ee left leg and left hip knocking him to the ground. Other ee called 911 and ee was taken to the local hospital 3 miles away. The hospital is small ultimately ee was transferred to St. Johns Mercy St. Louis for further testing and care.
Injuried was walking from office to motor barn slipped on ice and hurt his right shoulder. He did not miss any work until 5/8/14 when he went off for surgery.

Employee was rolling a 750# steel beam over to check a bolt hole pattern when the beam fell onto his right foot from a distance of less than 2'. Employee steel toe collapsed on his foot from the weight and broke his big toe.
Employee was cleaning build-up from pulley when his right arm was caught between the pulley and the conveyor belt causing fracture to left hand fingers, arm and scapula.

Injured employee was carrying a crib block in the 7 Right track entry when he felt a sharp pain in his abdomen. A pre-existing cyst ruptured causing the pain. Contact was not established with the employee until 2/19/14 to confirm a work related injury.
Employee barred down & then was in the process of loading the holes, when a rock contacted his finger, causing a laceration/avulsion to his R. index finger tip. No sutures were required, but restrictions were issued.
While changing bucket teeth for a shovel, a small piece of metal broke off the tooth and struck ee in the leg. The result was a 1/4 inch cut. The cut required no treatment at the time of injury. On 6/11/15, the ee had a procedure to repair a narrowed artery under scar tissue. The damaged is the result of the injury that occurred on 2/11/14.

Walking outside along the shop building to unlock diesel fuel tank. Slipped and fell due to snow and icy conditions. Hurt his left ankle.
Climbing up the ladder to cab on the Terex T-40 haul truck, missed a step while climbing and slid down the ladder to the ground. Was about 1-2 feet up when he slid down the ladder.
EE's foot slipped getting into the skid steer. The downward momentum caused him to lose his balance due to not having a full three points of contact. He reached for the roof of the equipment but was unable to grab firmly to the roof. He twisted around and fell to ground while his foot was behind the bucket. He broke his tibia and fibia in the fall.
Employee was operating a mini trac on #2 B supply road when he ran over a rock in the road which caused him to jam his head and neck into the roof. He kept working till the doctor put him off work on 3/11/2014.

Employee was operating H-116 CAT 365 loading a haul truck when the machine jerked and caused a pain in his neck. He left work at 5:15pm which is the normal ending time. He did not return to work on 2/11/14. On 2/12/14 he came to work and tried to perform his duty as an operator but stopped after one hour of operating the machine due to pain in his neck.
Victim was cleaning a coal spill inside Prep Plant using a shovel and wheelbarrow when his right shoulder and elbow started to have some pain in them. Victim did not miss a scheduled work day until 03/13/2014
Injured employee stated he was changing belt rollers, tripped and fell on a rock causing a laceration to his right hand.
During belt move in 22D, between 54 and 56 block, he picked up a piece of structure with a roller in it. The roller shifted pinching his left middle finger between roller and structure resulting in a fracture to the distal phalange. No issue with training, PPE, mining equipment or company rules.
Employee was running stage loader cable out of a wooden skid, the skid broke free striking the employee in the left knee. PPE was not a factor, mining equipment was not a factor, mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may be a factor.
Crew of three were moving refuge chamber with a scoop, the chamber was hooked in the scoop bucket with the winch rope hooked onto a 3/8 inch chain which was on the top of the chamber in a V shape so the chamber could be lifted up. After the move the bucket was lowered and the winch rope broke striking the employee in the lower right arm with the rope and hook.
Employee was climbing into a rock truck. He felt a pull on his left leg. He injured his Achilles heel and may require surgery.
Bolt man went to walk past the bolter after hanging his curtain and tripped and fell into bolt machine.
Employee was loading 12' track rails onto a flat car for transportation when he caught his right hands, middle finger between the rail he was loading and the flat car frame.
IE was rotating a tire manually on a 854K rubber tired dozed while standing on the push arm block when he slipped and caught himself. IE began to feel pain in his shoulder. 2-13-2014. IE was seen at the orthopedic specialist where it was determined that there was a severe rotator cuff muscle tear that will require surgery to repair making this a lost time injury.

End of shift EE was dismounting Kress coal hauler. His right foot was on the ground and his left foot became stuck on the step. EE twisted to free left leg, which is a prosthetic device, and twisted his right knee. Required ice and elevation and missed two shifts.
Employee was servicing a front-end loader. While climbing the vertical mounting ladder, employees hands slipped off the handrails and he fell backwards to the ground, landing on his rear end and hands. Employee's gloves were oily and he was carrying a half inch breaker bar in his right hand.
Removing sand from malfunctioning truck tarp, spring loaded tarp arm struck miner in face when enough sand had been removed to release tarp.
Subject was attempting to crush a piece of rock that was lodged in the Belt Feeder. He said, he was busting the rock with a sledge hammer and he went to move it with his left hand, the rock broke crushing his left hand.
Loading structure into scoop bucket smashed thumb between structure and bucket. Became lost time on 3/6/14
While removing transmission from D9L fastened transmission to the boom of the mechanic truck. After attempting to loosen the transmission with the boom, employee stepped forward to see why transmission would not come loose. The transmission ""popped off"" striking the employee in the chest and knocking him against the side of the mechanic truck, resulting in six fractured ribs.
Employee had an exacerbation of his personal medical condition, refused ambulance transportation and left property. On 3/21/2014 we received notification from our workman's comp carrier that a claim has been accepted for a fracture to the right knee.
Victim was walking across snow/ice covered ground when he slipped and fell injuring his back. Victim did not miss a scheduled work day until 02/13/2014.
The employee stated he was walking to get on his shuttle car when he stepped in a hole and twisted his left knee causing a strain/sprain. The employee continued to work until 2/24/14. The employee began missing work on 2/25/14 while under doctors care.
Employee was moving a rock off the top of the continuous miner when the rock slipped striking Employee's finger. Employees finger was infected and has been treated with antibiotics. Accident was not reported timely by Employee to the Mine Management. Investigation has been pending.
P&P contractor was knocking water line apart with anchor pin. The water line sprung apart catching his middle finger between the 2 joints of waterline resulting in a laceration requiring stitches.
Had been sweeping community roads with a John Deer farm tractor with a sweeper attachment and was bringing it back to the shop. The tractor stalled on the hill of a mine access road and traveled backwards into a drain. Was wearing seat belt but received cuts on the head that required staples and stitches.
While unloading batteries off of a flat car EE lifted a battery while turning around and strained his knee. On 2/27/14 I was notified by EE that he is going to have surgery on his right knee and it will become a lost time injury.
He is a newly trained miner roof bolter who was using an air drill in an outby location. He broke company policies by leaving suspended loads unprotected by removing the bottom drill steel and leaving the top three in the hole without being held. The steels fell out of the hole striking his left thumb. He was wearing metacarpal gloves.
Moving rock ground was froze frost broke machine flipped operator broke arm.
EMPLOYEE STATES AS HE WAS WALKING TO MANTRIP HE STEPPED ON AN UNKNOWN OBJECT, STRAINING HIS RIGHT KNEE WHICH RESULTED IN A TORN ACL / MCL.
Employee slipped on ice in parking lot while walking to the office building. He tried to catch himself falling by grabbing the banister, but jabbed his ribs into it as he fell.

Employee was back dragging a drill bench when he backed over an embankment and roll his dozer over onto its side.
Employee states he backed over a rock operating #20529 D10 dozer and was jarred straining his left leg and lower back. Employee was examined by Dr. at Lincoln Primary Care on 2/11/2014 and taken off work for a strain to his lower back.
Employee states he hit a dip in the road at the bottom of the 495 Shovel ramp causing pain to his lower back. Employee was examined by Dr. and taken off work and referred to a neurosurgeon.
EE WAS STEPPING OFF TAILPIECE AFTER GETTING ROCK UNSTOPPED AT THE HEAD AND TWISTED HIS LEFT KNEE. EE DIDN'T SEEK MEDICAL ATTENTION/OR MISS WORK UNTIL 2.7.2014.
Picking up trash at 50 block Main West (Belt to Track crosscut) and tripped on crib tie, falling on his right elbow causing a contusion. Believes he hit his elbow on his SCSR. No issue with equipment, training, or PPE. Company rule of making work area safe was in the process of being conducted.
Employee was working with 2 other ee removing head pulley wiper & mounting bracket on loading bin. When lifting mounting plate off bin employee felt sharp pain in left groin. thought he had just pulled muscle. Pain still present next morning so employee was sent to doctor and was diagnosed with hernia. Employee under went hernia repair surgery in Fri 2/7/14.
Employee was prescribed physical therapy for his shoulder. We were notified of this physical therapy on 3/4/2014.
Employee was re-entering a miner cable into the machine when he felt a sharp pain in his back.
Employee stated that he was picking up two buckets of miner bits, turned to go to scoop, and stepped in hole. Twisted back and hip.
Bending roof bolt in hole to straighten, bolt spun in hole causing him to fall. Pain in left shoulder, arm and hand.
Employee was struck on the heel of his right foot by a hollow concrete block. The block fell from a wall that he was building. He received a contusion to his right heel and taken off work for a period of (2) days.
Employee was sweeping off side West C belt and stated that while he was sweeping, he had a pain in his right shoulder. He has been treating with physician since time of injury and due to his restrictions issued 3/3/14 - we can no longer accommodate his restrictions as of 3/4/14.
Employee was helping remove a drip pan from a belt discharge when part of the drip pan hit the employee in the back of the head.
Employee was dismounting a Cat end loader. As he descended the ladder he slid his hand along the hand rail, when he stepped to the ground he over extended his reach and ""pulled"" his left shoulder.
Pulling wires thru conduit; twisted back.
Employee stated that he was backing the continuous miner out of the # 9 heading with his back to the miner when the cats either slipped or he split the cats, it was at this time the tail boom of the miner struck him in the middle of the back knocking the wind out of him. He was complaining of soreness in his back and a tingling sensation in his right leg and foot.
Icy weather condition. Re-setting the truck scale, slipped & fell on ice, slid down & landed on right butt cheek. Pain in right butt cheek & up toward kidneys; also going down to top of knee & front of shin.
Employee was picking up a 15HP pump when he felt a muscle pull in his stomach. The employee saw his family doctor the following day and was diagnosed with a hernia.
EE was lifting cable on corner to hang miner cable one more time when he felt pain in groin.

Employee was walking when he tripped and fell and felt a snap. Employee went to hospital with a broken leg and will remain off work for an uncertain period of time.
Employee lifting 130lbs drill bit to back of truck and strained his back.
Driver was dumping a load of Coal and while the box was raising it became unstable due to frozen coal stuck in the trailer and an uneven surface which caused the truck and trailer to tip over.
On 2/01/14 the employee reported that he had re-injured his Right Shoulder a couple of months ago pulling a dust line. No injury report was made at the time. The same shoulder was previously injured in 2011. He stated the MRI showed a Rotator Cuff tear which would require surgery. On 2/5/2014 he was taken out of work by his doctor pending Right Shoulder surgery.
Employee was operating the tailgate drum cutting toward the headgate when he stumbled over the 335 shield relay bar causing him to fall striking his left knee. He continued working until May 7, 2014 at which he time he had his knee scoped and began losing time.
Worker had rubber boots on. Some dry dolomitic got inside the boot, his feet were sweating and he had a blister on his heel. resulted in lime burn.
After backing a set of rail cars in on track #5, an employee was dismounting steps on the rail car when his foot slipped, causing him to have to catch himself to prevent falling. In the act of catching himself while holding onto the handrail, the employee felt a sharp pain in his lower back.
Employee was shoveling built up materials on the catwalk around cone crusher. A baseball size rock that was on the lip of the basket surrounding the cone feed, vibrated off the lip and fell onto the employee. The rock struck employee right in the middle of his back while he was bent over, shoveling.
Standing on ladder, EE was tightening bolts when his right calf erupted in pain.
EE was working on a concrete dock which rolled causing the employee to fall. The EE worked the rest of his shift & worked the next day. On March 3, 2014 EE reported a sore back but was unsure of the exact. EE took a week of vacation to rest his back. On March 10 EE told employer that the fall was the probable cause of back injury & sought medical care on March 11, 2014.
While climbing ladder to enter his maintainer, employee reported straining his lower back.
While working on the Stone Plant material chute, employee felt a pop in his right knee. He continued to work rest of shift.
A laborer injured his left shoulder while moving conveyor idlers from the back of a truck to the manlift basket. Additional diagnostics (MRI) positive for rotator cuff tear now make this reportable.
Employee was crawling under the six-wheeler to check a rear tire when his flashlight fell striking him in the right eye. The contusion resulted in blood pooling in the eye causing temporarily impaired vision which required prescription drops and monitoring.
On 2/25/2014 Employee was walking & slipped on snow, twisting left ankle. Went to medic & Doctor-full release issued. On 3/11/2014, employee went back to doctor/MRI was done & read-revealing an upper hairline fracture to his fibula. Doctor issued restrictions
Employee lifted up chocks from side of wheel of forklift, went to toss the chocks in the bed of the forklift, chocks caught on frame causing chock to be pulled out of the employee's hand falling on top of right foot.
Employee was beginning work on hood of Mack truck. Stepped up on bumper, foot slipped and he fell back onto steel bumper of service truck which was parked in front of Mack truck.
Employee was replacing a bearing. Was on his knees, got wet and got lime burns on his knees
I was swinging a 16lb sledge hammer trying to install collets on a 993K wheel loader. I was in an awkward position when using the hammer.
At around 7:00pm control room operator was replacing them belt on a screw. She was positioned on top of the cover for another screw. While opening the guard she lost her footing. When she made impact with the surface below approximately 2 feet she struck her right knee on an old pipe that is no longer in service. Currently at home waiting to return to the dr. for an MRI.
Employee was trying to prevent equipment door from closing, so he put his hand in between, and got his right hand smashed with the door, and his middle finger got broken.
Miner was rolling a rock towards a scrap pile when he picked it up to place in the pile he felt a pain in his right abdominal area. Pain was a 3 on a scale where 1 is the lowest. He finished his work shift thinking that rest that evening would take care of problem, but still had pain when he returned to work the next day.
Was putting a crib under feeder to help get on low boy, he slipped on ice & felt strain in shoulder. This employee did not miss any work until June 4th, 2014.

EE was climbing down a ladder and after he reached the ground he turned and slipped on some snow that he just brushed off a railcar. This incident happened in our 500A silo. EE fell landing on his tailbone. He was taken to the local hosp. and was released the same day. At this point he is on light duty for two weeks as per doctors orders
EE was attempting to straighten the worm gear drive that was just placed on a pallet. While attempting to do this, he felt discomfort in his L wrist. He was evaluated by a Physician, a splint was ordered and he was given restrictions.
The employee was using a sand blaster when the breather hose got stuck. The employee pulled on the hose slipped on the grit under his feet and lost his balance and lost control of the nozzle. Sand blasting grit struck the employee above the right elbow causing a 4 to 5 inch hole in is bicep.
Employee was standing on conveyor of the powerscreen, putting tools in bucket of Bobcat. Employee slipped and fell off of the powerscreen, about 3 ft in height. Employee did have safety harness on at the time causing employee to be pulled back towards the Bobcat bucket. Employee's legs hit the Bobcat bucket causing a laceration to left lower leg.

The employee was working on the 6th floor of preheater tower building scaffolds. The employee stated that he was experiencing pain in his legs and thought nothing of it. Employee stated, when he got home his wife gave him some Tylenol. He stated that when he woke up his whole right leg was numb.
Employee was swinging sledge hammer when he experienced knee strain behind left knee as he pivoted.
Employee was loosening a port cover with a pipe. In the process a piece of welded rod broke off and struck him in the nose.
Employee was walking to get to loader, slipped on a patch of ice and twisted knee. Employee hyperextended knee and heard a pop. Went to doctor. MRI Scheduled for 2/25/2014.
The employee was lifting a skirt board back on to a conveyor system that was being worked on. The skirt board was stuck in the mud so the suction on the object made it heavier than usual. The employee twisted while lifting and pulled a muscle in his lower back. The conveyor that was being worked on was locked out.
Employee dropped a steel I-beam on right foot breaking bone in great toe. Employee was placed on restricted activity. At follow up evaluation on 2/24, restrictions were extended and the company can no longer accommodate the restrictions placed upon the employee by the treating physician. Lost time as of 2/24/14.
Employee slipped on ice and hit his left knee on a nearby trailer.
Two employees were injured while performing maintenance on our primary feed hopper when another employee mistakenly dumped sand and gravel on them not knowing they were performing maintenance in the hopper.

Employee was walking around in snow at shop area loading corner bits for dozer, slipped in snow when he hooked chain to bit, causing a strain to the low back
The roof bolter operator was drilling a hole when his forearm was struck with a wrench. The resulting impact caused a simple fracture of his left forearm.
employee slipped on a scaffold and fell approximately 10 feet landing on a horizontal scaffold pole in the sitting position.
Employee was looping up belt. When he began to pull on the belt dog it slipped off the belt and struck him on the left forearm.
Employee stated that he was getting out of the mantrip when he stepped on ice/snow and slipped causing him to fall to the ground and twist his back. This resulted in a back strain. Employee continues to work, but was placed on restricted duty and began receiving medical treatment on 5/6/14.
Employee was using a torch to melt ice when the torch turned in his left hand and the flame went across the back of his right hand. He suffered a 2nd degree burn.
EE Was helping another employee ""walk"" a 350lb hammer (for a rotary mill) onto a pallet when he heard and felt a ""pop"" in his right shoulder. EE reported the incident to his supervisor on 02/14/14. EE was sent for medical evaluation and diagnosed with a Right shoulder strain/sprain, prescribed PT and returned to work with restrictions.
Employee had been using an 18"" long pry bar removing ductwork. Employee complained of right arm/shoulder pain approximately 30 minutes after completing this task.
Fall on packed snow/ice while doing a 360 inspection after fueling mobile equipment. Injury to left hip/hand initially self-treated. Taken to clinic 2/13/14, x-rays negative, no prescription. Follow-up visit on 3/13/14 employee given script for muscle relaxant for back causing recordable accident.
Injured was changing screen panel in vibrating screen. The vibrating screen was de-energized and locked out. As injured exited the screen deck the injured pulled on the drive mechanism vee belt. This caused the sheave wheel to rotate pulling his right hand between the belt and sheave wheel resulting in a laceration to the palm and fracture of the 5th finger .
Employee was splitting rock and while manipulating the stone to split he felt a pain in his left shoulder and in his groin area.
While lifting a 4'x8'x1/2"" plate of steel into position in the prep plant elevator chute, the plate came dislodged from the chain and hoisting equipment striking the employee in the left foot. Two fractured bones in his left foot.

On Feb 11, 2014 employee was climbing haul truck ladder and felt something pull in his right shoulder. EE had a first aid doctors apt on Feb 18, 2014. On follow-up visit on April 18, 2014 was placed on restricted duty.
The employee was stepping from the walkway of the Komatsu WA800 wheel loader, onto a step to gain access in order to change the right tilt cylinder hose. He lost his balance when taking the step and fell 11' to the ground. He suffered a laceration above his right eye, a broken elbow and facial fractures.
Employee was stepping from an elevation off his trackhoe and twisted ankle when left foot contacted pea gravel and stick.
Employee was moving a storage rack when the top horizontal rail (that is attached to the vertical support with a hinged mechanism) slipped off its base of support and came down. Employee's finger was pinched between the vertical support and the top horizontal rail causing a laceration and fracture.
Employee was walking outside the Maintenance Shop when he slipped on ice catching himself with his left arm on an adjacent truck straining his shoulder. He is working until shoulder surgery which is scheduled for November 3, 2014.

Doctor prescribed pain medication and gave restricted duty instructions.
Employee was descending staircase on a Hitachi 5000 haul truck. When he reached the landing in the mid-point of the stairs, he slipped and twisted his knee. Employee was evaluated and given restricted duty.
Employee was performing fireboss duties, as he was walking past a belt drive location he slipped and felt pain in his knee. He has been placed on restricted duty.

Employee was using a roof bolting machine to install chain hangers in preparation for a belt move. While pushing a steel back into the drill hole, the steel suddenly angled off to one side. As the employee reached to straighten the steel it recoiled back, striking his left index finger. The impact fractured the finger.
Employee squatted to perform safe back lift, and felt pop in left knee, and subsequent pain.
While working on a floating pump assembly the miner was struck in the back with the boom of a excavator. He has five broken ribs and a partially collapsed lung.
Employee was carrying concrete block and twisted his left ankle resulting in fracture. Restricted Duty
Employee working inside Mill 7. Hydraulic boom moved unexpectedly and struck employee in chest. Employee was diagnosed with a chest wall contusion.
The roof bolter was being trammed from the side controls. The accident victim told his partner he would take over tramming from the rear deck. As he entered the deck his partner stepped away from the side controls. His glove hung on one of the side control tram levers causing the bolter to lunge. The victims left foot was caught under the rear deck spraining his ankle.
Injured employee was removing arm guard from H6800 spider, using a sledge hammer to break arm guard loose. Employee went to the doctor on 8/29/2014 and he recommended surgery to the left shoulder
walking on decline when felt rt knee pop. diagnosed with fractured knee cap and torn ligament
Employee was loading bottom stands. Two stands were stuck together; as he pulled them apart and started lifting a stand up he felt pain in his lower back.
Employee utilized a shovel and scraper to clean out pits under drum rollers. Employee noticed pain and soreness in his lower back in the evening after his shift ended. Employee reported this the following shift.
Employee was throwing miner cable out of roadway and felt pain in lower back. He continued to work and did not seek medical attention until 02/18/14, where he was seen at LRMC where he was treated and released. On 02/19/14 he was seen at St. Mary's Occ. Health Center where he was put on restricted duty.
While on ladder, EE was pulling slack out of the new skirting being installed on the conveyor belt #1 and strained or pulled lower back. Continued to do light duty work rest of the day.
Labor Finders temporary employee: Placed hand toward a moving pallet and finger got caught and pinched. Resulted in seven stitches to the right ring finger.
EE was conducting a building inspection when she stepped on a man hole cover causing her to fall to the ground. EE was seen by first responder and then taken to the town to doc office to be checked. EE was placed on restrictions and released back to work. EE hurt her right ankle and right wrist.
Associate was exiting the primary crusher area after replacing toggle plate and seat. While crossing over an I-beam, he tripped and fell fracturing his wrist.
Employee was stripping the inner layer of cable when the stripping knife slipped and lacerated his left hand just below the little finger. The laceration required sutures to close.
The man was crawling in the section when he reported low back pain.
Employee exited tractor trailer cab and stepped on ice resulting in a sprained right ankle.
Employee was clearing rocks from the coke hopper. He cut a hole in the side of the coke hopper and reached into the hopper to pull a large rock out, when the rock started rolling and fell on his left index and middle finger breaking both fingers.
Employee stated he was unchoking a rock reclaim tripper when he slipped and hit his rib cage on a piece of angle iron. He received one fractured rib and was placed on light duty.
Employee picked up one end of a thread saver for an overburden drill and felt his left calf muscle pop behind left knee

While climbing down from DZ-7 (dozer) employee stepped to the ground and back started hurting.
E was lifting bundles of split bolt plates by hand transferring them from a loader bucket to a shipping container. while lifting bundles he felt a ""pop"" and pain in his right wrist. E had right wrist put in a splint and is on restricted duty.
While assisting fellow employee trying to hammer out a 3/4"" dia. bolt out of slot with 2 lb. hammer, hammer hit left middle finger and broke finger.
EE caught safety vest in spinning drill auger and was entangled in auger. EE received bruising to ribs and right bicep.
Employee was material into the open bed truck. While he was operating the load out controls, the wind blew material coming out of the load out chute in his direction. Material got behind his glasses and into his right eye causing irritation to develop.
Employee was struck in the face by a hook on a pulley attached to an overhead crane.
Setting a motor base into place with a come along, the hook on the come along sprung loose cause the employee to fall face first into a 5 gallon bucket causing loss of consciousness and a laceration that required stitches to close.
Employee was loading a truck out of silo 8. The gate was stuck open and material did not stop flowing. Employee went out of the office, thru the center bay door and up the ladder to close the gate. As he was coming back down the ladder he pulled/strained his right calf muscle.
For about three days the EE had been working in a confined area on his knees bent over at times using a Mag Drill to place steel plates to the wall and to secure the adjusters in the holes.
Employee was operating a shuttle car when it hit a hole in the road way causing the employee's head to strike the canopy.
EE was in the process of descending the ladder of an off-road haul truck during pretrip inspection and slipped on step (fall to below). EE sustained fracture to left leg (unspecified). EE stated buildup of mud on his boots caused him to lose traction with step surface.
Employee went to get a sample from the alkali bag house. He got the sample and was walking back when he tripped and fell fracturing his left wrist.

Return to work with restrictions the following scheduled shift.
Slipped down last 2 steps on the ladder. Tried to catch himself. Got hand in hot rocks, some went in his glove.
EE was removing material from the crusher discharge area and he picked up a large piece of material and attempted to break it by throwing it on the ground. When he went to throw it down he felt his nerve get pinched in his back. He knew exactly what had happened to his back because he has injured his back the same way a couple of other times in the past (prior to GEO).
EE was attempting to move a stored hydraulic cylinder from the T-tower. Partner moved one end and the cylinder slipped from the edge of a raised (12"") surface. The end of the cylinder struck EE's L foot, resulting in a fracture. EE returned to modified work on the next scheduled shift.
When turned on, the air bubble in the washdown hose caused the hose to twist in the hand of the employee and the nozzle attachment band cut the employee's right hand at the base of his thumb.
walking from outside office steps onto pavement. Slipped in black ice. Sprained right ankle/foot
The employee was bolting the back with a jackleg drill. As he was backing out of the hole the steel puller bounced up and released the steel which caused it to fall striking the employee between the thumb and forefinger. This caused a laceration which required sutures. The employee was disciplined for using defective equipment.
On 20-Feb, employee was walking to her car in the employee parking lot when she lost footing on gravel, causing her to fall to the ground. She tried to catch herself with her extended arm causing injury to her elbow. The employee sought medical treatment on 24-Feb and received restricted duty. The employee did not report the incident or the medical treatment until 26-Feb.
While an IMTR employee was unloading a drill steel from a delivery truck, the forks of the lull hung up on a cross member, when it released it pushed the drill steel onto the Fred Weber Inc. truck driver's foot.
Employee was switching out an empty barrel of grease with a full barrel that weighs approx. 120 lbs. As he moved the barrel to the ground he felt a stretch in his lower back but no pain. The pain started on 2/21/14 in the morning and at that time employee requested medical attention.
The man had finished installing a row of roof bolts. While preparing to advance the roof bolter, a piece of roof scale fell and struck the man on the knee.
Two employees were removing return roller on C1. When the roller cleared the hangar brackets it slipped out of their hands striking the right middle finger of one employee's hands against the concrete pad. Contusion to tendons/ligaments.
Employee was unloading steel plates from pallet on forklift. one of the forks slipped and hit employee in right knee causing cut and swelling. Employee was taking to quick care and treated. Returned to work with light duty on 2-20-14. Resumed to full duty as of 2-26-14
Injured Employee(IE) was removing raw mill spout cut bolt and was driving out bolts to remove the spout. The spout moved and made the alignment bar come in contact with his left and the hopper of the raw mill causing bruising. Subsequently a knot or cyst has come up requiring more attention.
Fell over cable tray installed on the ground hitting left knee
Fall from the 3rd rung on a 6 foot ladder. Ladder was placed on an uneven surface and had an improper hinge repair.
Employee pinched finger between a pry bar and a work table.
EE fell on wet surface when going outside to fill forklift propane bottle. He indicated was alright and refused medical treatment. 2-24-14 EE requested to see doctor for back pain and was sent to medical provider. Sprain, lumbar sprain. modified duty with 30 lbs lifting restriction and ok to drive forklift. Follow up appointment on 3-17-14 with doctor.
Employee was rigging a hydraulic cylinder using a chain fall. When the load became suspended, it shifted to one side trapping the 4th finger on his right hand between the load and the chain.
Employee standing on drill table slipped and fell backwards hitting his head.
Employee using a digging bar during clean up, impact to right ring finger, resulting in a fracture in the tip of the finger and lacerations. Employee is on restricted duty.
Employee emptied two 5-gallon buckets of sand. While carrying the empty buckets back to the lab the employee felt a sharp pain in her right shoulder.
On 2/18/14, a temporary contract employee complained of shoulder pain. He could not specify a particular date or time the pain began, but said he thought it was related to his activities at work. The temporary employment agency was contacted and the employee was removed from the worksite.
EE was examining the cleaning cells at the mill which sit on a slightly elevated floor. The floor is constructed of metal grates and there is a 6"" step up to the elevated floor. When she left the area she misjudged the step as she was leaving the elevated floor. This caused her to fall on her knees resulting in a L. ankle sprain, and a sprain and a laceration to R. knee.
3 employees setting up an ACI to burn pile, needed help from excavator to extend the last 3'of burner pipe. Excavator operator while clearing path to ACI grabbed a small stack of limb debris in his way and pushed it into the pile. 4"" by 6' long limb sprung out of pile striking employee on the ground 25' away. Breaking forearm (Ulna) and c1 vertebra.
Employee had arrived for work and was leaving the parking lot to come in and slipped on the snow and ice injuring his left knee.
Employee felt a popping sensation in her elbow when trying to open a valve. Employee went on restrictions on 02/27/2014.
Employee was lifting the cab of a skid steer loader to get filter numbers. As he was lifting the cab he felt a pain in his abdomen / stomach area. It was determined that he had a hernia. The hernia was repaired and employee was placed on restricted duty.
Employee slipped while installing HP filter on a shovel.
Employee was checking a hydraulic leak and slipped in the park ditch as he was going under the truck. His left rib cage area contacted the truck. Employee stated he was OK when he reported this to his supervisor. The next shift day, he indicated he was sore and would like to be checked out.
Employee alleges that he felt pain in the elbow as a result of working in Shipping, while helping out in that department the week before Christmas 2013 . When it did not go away and got worse he reported it 2/17/2014.
Injured was observing another employee disconnect 2"" ""make-up line"" from suction line on primary fines pump when he got sprayed in face with hot material which blew out of pump line.
Employee used a 11.5 lb dead blow hammer to loosen a stuck wheel off the 686 truck. Employee swung the dead blow hammer about 5 or 6 times and felt discomfort in his right shoulder and arm.
Employee was splitting a stone and lowered the head of the splitter instead of the cut cycle. The left index finger got caught between the dust collector and the stone.
Employee missjudged the single step along a walkway and fell which resulted in a laceration to his right palm and required sutures to close.
Injured was in process of removing ""make-up water line"" from suction line on primary fines pump so as to add water. 2"" make-up line broke off at valve overhead causing line to fall when hot material blew out of 2"" opening and sprayed injured' s face and neck area.
EE was shoveling snow in alley when slipped on snow covered ice and fell onto his back. Suffered an ""acute lumbar sprain"" to his back. Treatment was take over counter ibuprofen and perform exercise and walking at home for treatment. Return 2-27-14 follow up.
Two employees were injured while performing maintenance on our primary feed hopper when another employee mistakenly dumped sand and gravel on them not knowing they were performing maintenance in the hopper
While walking down stairs operator twisted his left ankle falling forward down to the next step.
Employee was cleaning return rollers on a conveyor belt with a poke bar. The conveyor belt was running. As the employee was scraping, the poke bar caught on the roller and jerked his right hand up jamming it against the frame of the conveyor belt. He broke two bones in his hand and received a laceration.
Injured was investigating a non-functioning heater in the pump room when he bumped a pipe, causing dust to fall and enter his eyes.
Employee was walking on the south side of the cooler building near the IKN fans when his right foot slipped on the snow. The employee twisted his left knee. The employee has a sprained left knee and has been placed on restricted duty.
Employee and partner were carrying an empty 100 pound propane out of 44 tunnel. He switched his carrying hand from right to left and felt a pain in his mid back. He was diagnosed with thoracic sprain.
EE slipped on snow covered ice, felt pain in right shoulder, EE had surgery 5.8.14.
Employee called to report that on Friday, 2/14/14, @ 1000 am she was getting a sample for a customer using a 5 gallon bucket & when she went to pick up the bucket to place on tailgate of truck she felt a ""sharp pain in the front of the right shoulder that shot shooting pains down the arm to the fingers & up the neck"".
Employee injured back when struck by pallet of plywood when forklift tail-swing bumped into the stack on opposite side. Soreness only symptom. Internal First Aid report was submitted. Returned to work next scheduled day. Nearly two months later, he saw a doctor with soreness in back. Occupational doctor released employee to work with lifting and bending restrictions on 4/10/14.
Employee strained his right shoulder while relocating a magnetic drill.

Employee slipped on a walkway causing injury to his right arm. Employees arm did not get any better and employee went to the doctor on March 12, 2014. He was then placed on restricted duty.
Employee was beating on outside of metal coal hopper with sledge hammer due to coal being frozen. Sore arms, shoulders, thought pain would go away, it didn't. Investigation started 3/5/14. Seen by physician 3/6/14. Placed on restricted duty back to work and is currently working on restricted duty, no time lost.
Employees were removing a take up pulley with comealongs. One of the rigging points had a slight side load on it causing the pulley to drift when the final bolt was removed. The pulley contacted the employee's left thumb in a pinch point on a channel iron causing a compound commuted avulsion.
An employee was walking down a ramp and twisted their knee. The employee was transported to the hospital where they were released to restricted duty.
Employee was climbing off a drill. He slipped and grabbed the handle but still fell to the ground. The drill, including handle, had grease and oil on surfaces contributing to the slippery conditions. Employee has a strained right ankle resulting in restrictions to walking, climbing and crawling which requires the use of crutches.
While fussing pipe, the pipe slipped and caught finger between pipe and machine.
Employee was walking down a stairway in the coal mill area when his foot hit the edge of the stairs which was smooth and worn down (slippery). His left foot stayed in place and his right foot slid down; he slipped and when he fell most of his body weight landed on his left leg causing a fractured fibula.
Slip & fall. Initial incident - from snow packed over. Employee fell on right elbow straining elbow. Employee continued to work. Elbow became more painful from the repetitive motions of the work causing more strain to elbow.
Employee was directing a tractor-trailer flat bed truck of drainage pipe in the A-Frame building. While directing the truck to move forward, the truck drove over a bump. Apparently there was a loose 4 foot long 2 x 4 on top of a section of the piping which bounced off of the load striking the employee in the left thumb. The employee suffered a broken LT thumb as a result.
Injured worker stepped out of pick up truck and strained / twisted right knee. Reported accident on 3/7/14. Doctors appointment on 3/10/14 and was placed on restrictions.
Fixed Plant Mechanic was helping Mechanic remove a 200 pound bearing from a pallet. During the removal process, they dropped the bearing and it landed on top of Plant Mechanic's left foot, which resulted in the fracture of 3 metatarsal bones.
EE requested medical treatment on 2/14/14 and said he injured his back on 2/10/14. Doctor determined injury to be non-work related. EE went to his personal doctor and his injury was determined to be work-related. EE was sent to a third doctor on 2/25/14 and his injury was determined to be work related. EE was placed on restricted work duty and this became reportable on 2/25/14
Employee slipped on snow/ice and twisted left knee and was given restrictive duties.
Employee was inside material chute to replace wear liners. While leaning the new liner against the wall of the chute, the right middle finger was caught between the unfastened liner and the chute wall. Injury required stitches.
While employee was cutting nylon bands off of cable bolts the belt knife he was using struck his right leg in the calf causing a puncture wound.
Employee was performing a walk around inspection and slipped on a slick surface. This injury resulted in a sprained ankle.
Illness - chronic knee condition. Employee felt soreness in knee by the end of the shift. Knee was hurting for several days.

Climbed out of equipment. When reaching the ground turned to walk and the ground was covered in snow and ice and stepped into a rut and rolled ankle.
Worker was splitting a stone using a hammer and chisel when the stone fractured trapping his gloved hand and the #4 finger of his right hand causing a open fracture of the finger.
EE was walking between work locations, slipped on ice and strained his left knee
EE was standing inside discharge chute when foot slipped causing knee pain, dr prescribe no climbing restriction until MRI determination of injury.
A Maint. operator was checking on repairs made to railcar loadout pump. The loadout operator prepared to load railcar by lowering a 4"" alum pipe inside railcar. The Maint. operator didn't see flow going into railcar and was told manual valve was closed. Upon opening valve, the 4"" pipe came out of railcar striking the maint operator's fingers that were on the valve.
On February 6, 2014 at approximately 10:00AM, EE was working at the bucket wheel on the West transfer conveyor with 2 co-EE's. He slipped on ice and fell on his right side. He immediately felt pain in his right elbow and his lower left back. He reported this to company immediately. EE was driven to One Medical for evaluation.
EE slipped on patch of dust covered ice from a leaking dust suppression line, straining his left knee. All scans were negative, EE's physician prescribed a knee brace to wear, medication for pain/swelling and placed on restricted duty - follow-up appt. scheduled for 2/14/2014.
Moving equipment through a curtain, stepped off sled (6"" high), to pogo a curtain down and twisted right ankle.
Employee was standing in the entry when he heard what he thought was a rib falling. He turned to move away and tripped over a lump of coal along the rib. As he fell his foot caught on this lump, preventing his foot from rotating with his lower leg resulting in a twisted knee.
Employee was working on the left side of a scraper kneeling down next to a mechanic truck. The operator of the truck honked twice and pulled forward not knowing that the other mechanic had stuck his foot in line with the rear tire. The edge of the rear tire ran over employee's right foot.
Turning water off on water storage tank, slipped on ice, fell and hit his head causing a concussion.
Employee alleged that he tripped on a some material and fell on his right arm while placing items on a forklift.
A Labor Finder Laborer took the water hose off of the pressure washer. He failed to release the pressure from the hose, and steam from the water line burned his right hand and wrist.
Employee was shoveling debris under conveyor belt in Roofing mill. He picked up a load that was too heavy, and the shovel tipped to the right and the load fell out of the shovel. He felt immediate pain in his right arm. Employee reported the pain the next day. He could not twist his arm or his wrist.
He was making spice on pinner cable, when his knife blade went through cut, cutting bottom of left thumb. (He received stitches)
Employee was using a spud bar to break up frozen material in the dust bin under lock out conditions. The employee reported shoulder pain shortly after, but continued to work as scheduled. EE continued to work his full duty until the shoulder was surgically repaired, after which he reported for restricted duty on his next scheduled shift.
Employee states he felt pain in his back.
Team member was driving the utility vehicle from the Blue House area to the office. While driving downgrade near the Recycle area, the vehicle began to slide and turned into the berm on the right side of the road. The front of the vehicle contacted the berm then rolled over onto the passenger side. Team member received a concussion and laceration to the left side of the head.
Employee was walking up a slight grade and slipped and hit his head.
Employee started walking towards 48 forklift when he slipped, tripped and fell over a pipe that was sticking out in the dark. He hit the ground cutting and splitting his left hand.
