No. 3 Hoist, pull muck off 5200 halfway up shaft hoist slowed down from 1700 to 500 FPM, computer gave pinion brake supply pressure fault. Found loose connection on pinion brake supply pressure transducer.

A non-injury incident occurred when the brakes to the slope car malfunctioned. There was no production at the time. A personnel (fire bosses only) were notified and removed from the mine. The primary and alternate escapeways were still available. No one was injured during this event.
On the evening shift of 3/30/2019 a roof fall was discovered in the #1 and #2 entries. The #2 entries serves as the return travel entry. Area was breakered off and made safe. The stopping line was set over.
A non-injury roof fall occurred at #5 belt entry intersection and crosscut over to #6 entry travel way or secondary escapeway. One crosscut out by survey station 12206 in the #9 entry.
An unintentional roof fall occurred in the Alma coal seam, #2 entry, #129 XC. The fall is approximately 6 feet high, 20 feet long, and 20 feet wide in the intersection.
Roof Fall was a continuation of an existing fall at #26 break on the #2 beltline in the #6 entry extending to the #27 break in the same entry.
Fuel leaked from a #2 Fuel Oil pump and came in contact with hot machinery causing a fire
Oil from the Kiln 6 drive gear sump contacted the kiln shell, exceeding the oil's flashpoint and started a fire. The kiln was shut down for 36 hours to replace the guard, which had buckled due to the heat of the fire.
Build up in the pulverized coal bin was exposed to oxygen during an outage and it started smoldering.
Change of weather caused immediate roof strata to loosen and break from surrounding top.
First intersection inby intake portal fell approx. 8 ft. high, 16 ft. wide, 23 ft. long. Fall did not affect ventilation, all employees were immediately evacuated from the mine. No injuries resulted from this event.
During routine inspection of the Crabapple Hoist, a deformation in the hoist rope was found approx. 25 ft from the car. The Hoist was taken out of service and the rope was reterminated. It is possible that the tension in the rope when the cars are coupled caused this deformation.
Roof failure from intake portal inby approximately 16 feet.
Roof fall on No. 1 Section; No. 1 Entry, 1 break outby face near survey spad 14357.
Third party trucking company's service pickup truck caught fire while driving through the facility at approximately 12:10pm. MSHA hotline contacted at 12:22pm. Occupants exited the vehicle and cleared the area. Fire Department was contacted and responded. Fire extinguished at 12:50pm.
At 5:30 pm the continuous miner intersected an uncharted gas well. The location of the well was 1 room off of an entry at spad location 3+45 on the 3 left section.
During the weekly examination of the Toney's Branch hoist it was found the hoist failed the required brake test, the hoist was removed from service and the brakes were adjusted.
The Crabapple hoist was taken out of service to replace the bearings on the hoist rope drum. During the bearing repair, damage to the hoist drum shaft was noticed. It is unknown what caused the shaft damage but it was subsequently changed.
A roof fall was discovered at 220 crosscut in the #1 entry 1 crosscut inby spad 5281. This was a continuation of a previous fall.
At approximately 4:20pm the main hoist would not reset, after a power failure and a blower fan motor failure. A call was made to MSHA at approximately 4:40pm. The hoist was back in service at 10:00 p.m. after repairs were made.
On 3/13/2019, the north-east side of the mine Impoundment experienced a loss of stability at the out slope in the area of sediment cell #2 between the top of the cap and the embankment. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
A roof fall was discovered on this date. The fall was located on the B-2 Mains in the number 5 entry at crosscut 44. The fall measured 25' by 18' and was 7' in thickness. The area was previously supported with 5' Hy-Tech bolts and 12' super bolts. The fall did not affect persons or ventilation. The fall was cleaned up and the area re-supported.
Operator was heading towards the lime silo, veered right causing ee to head down into the pit. Operator was going too fast when ee crested the ramp. As ee neared bottom of the ramp, ee took a left towards an empty dump. EE noticed an area of mud on dump & went through the mud to slow self down. Operator lost control causing the haul truck to tip over on its right side.
The Wana Elevator was transporting two employees to the surface when the north brake release solenoid shorted out at approx. 3:45pm, causing the employees to become entrapped until the coils could be replaced in the solenoid. Once the coils were replaced the employees exited the elevator at 4:26pm. It did not involve any equipment or compliance issues.
A roof fall occurred on the Productivity #1 track between 11 and 12 x-cut. The fall was approx. 23' in length, 18' in width and 8 to 9' in height. Area was bolted using 8' point anchor resin assisted bolts and 12' CC cable bolts. Area also utilized wire mesh and bacon straps. Cause of this fall was deterioration on roof due to weathering.
The elevator at BMX portal was out of service due to the door track for the bottom door breaking. The door track and alignment was fixed and the elevator returned to service.
A roof fall was found at approximately 6:15 pm on March 6, 2019. It was at the 1225' station (Crosscut 12) of #7 Main North entry. The fall was approximately 7' high, 20' long, and 40' wide. There were no injuries, and the fall did some damage to a stopping but didn't affect overall ventilation.
While testing the running program on the slope hoist, it was realized at app. 1:15 p.m. that the hoist would need to be taken out of service. After trouble shooting the software, problem was corrected. The hoist was tested several times and put back in service at app. 3:30 p.m.
There was an unplanned roof fall at the #4 1st south #7 Entry.
Roof fall occurred on HG9 entry #0 x-cut #6. An action plan was submitted to MSHA and approved.
A non-injury fall of roof in outby area occurred in the intersection at spad #611 above the roof support anchorage. The area was dangered off until additional roof support was added during the following shift.
Slope car was down due to an electrical problem.
The bottom door latch switch froze and interfered with the use of the elevator for over 30 minutes. The switch was replaced allowing the elevator to resume normal operations.
5200-206 E Cut 11; seismic event occurred during blasting resulting in failure of ground beyond support.
Dayshift crusher employees were cutting bolts to remove racking from a non-operation bins chute deck. They filled out burn permit, quit working 2.5 hrs before end of shift, ask night shift to inspect. Night shift found fire when doing final inspection
An ignition was reported in the G1HG section, located at survey station #31+84.59. The cause of incident was due to geological anomalies. A return spray was added to miner to prevent any further occurrences.
EE was changing the v-belts on the north log washer motor gearbox assembly. EE lost balance when wrench slipped off the motor mounting bolt ee was tightening and fell backwards through a 15 3/4 inch gap between the log washers. EE was conscious and talking and was told to remain in place until EMS arrived.
Victim was assisting with extracting the highwall miner cars. EE had been shoveling dirt/mud off the deck of the miner. EE was standing at the rear of the deck while a car was being extracted and for unknown reason head came forward and was caught between the car and a support post. Suffered crushing injuries to the head.
While preparing to perform side frame/track repairs on a 4100A shovel. When the relief valve was ejected from the jack. Employee was struck by a pressure relief valve in the lower left abdominal/waist area. The injury resulted in the death of the employee.
Mechanic was repairing primary feeder, guiding a bar with EE's left hand on top of the bar and running a hoist with EE's right hand. The bar moved unexpectedly and caught the mechanics left hand/fingers between the bar and the wall of the bin.
Employee was working in a 995 panel when the power got turned on causing the employee to receive an electrical shock and burns resulting in the amputation of left middle finger.
Employee was working with rail. The rail slipped off forks and hit employee's foot causing injury.
1 Driver and 5 Passengers were riding in a UTV during non-work related activities. The driver lost control of the UTV and the vehicle ended up on its side. The bump to the head is believed to happen at some point during this time.
Employee was exiting underground forklift cab when their foot allegedly became caught between the seat and frame causing them to fall backwards landing flat on back. The operator has completed its investigation of the alleged incident and has determined the incident may meet reportability criteria as set forth by the regulation.
A fully trained miner was putting a strap on the miner when foot was caught between the bolt plates and the miner. The injury was not the result of any compliance issues nor were any protective guards involved.
Tire guy climbed into wheel, slipped and smacked knee on the wheel. Initially diagnosed as a knee contusion but MRI revealed a meniscus tear.
Employee cut a tree. When the tree fell it struck another tree breaking the top out of the tree being cut. The top flew back and hit the employee.
Injured employee was installing roof bolts. The bolt was bent so that they could be installed. As EE tried to straighten the bolt EE felt a pop in EE's right shoulder.
The employee was shook up when a large rock was loaded into the back of the haul truck the employee was operating.
Employee fell off trailer while adjusting a lifting chain on a screen that was being loaded on a flatbed trailer. Employee was taken to hospital for broken ribs and internal injury evaluation.
Employee was installing cable bolts. Employee drilled up, leaving the steel in the top while holding it up with left hand. Employee was moving the next steel to put it in the drill pot. A rock fell from the top, striking employee's left thumb, causing injury.
Maint employees were removing a large cracked bearing from the dryer crusher shaft. As the outer race was moving outward, it released suddenly, and rotated. Due to its damage, the ee's hand was caught between the race and shaft, which resulted in a fractured left ring/pinky finger.
Employee was trying to remove a cable bolt that was stuck in the roof, while the employee tried to pull the bolt from the roof while reversing the head of the drill the bolt head struck EE in the left arm causing a laceration and shoulder pain. EE received three sutures on left arm.
A fully trained miner was opening a dust door on the miner when ee lost grip of the handle and right index finger swung back striking a bolt plate. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was hanging inline plug for the miner cable when the employee felt a pop in left shoulder.
Employee was exiting the loader and twisted left foot when stepping off of loader.
The employee was working on 6 East L/W Section delugging track ties. The employee had the rail jacked up, and placed a header board sideways to try and support the rail. When the employee went to remove the track tie, the header board kicked out and the rail came down the on employee's right foot. The employee did not violate any laws or company polices.
Employee was holding up the pan line to shield ""snake head"" coupler with a pick hammer in right hand. The relay bar was out (close to the snake head) and ee was using left hand to feel for alignment, for the relay bar to couple with the snake head. The pick hammer slipped, allowing left ring finger to be pinched between the end of the relay bar and snake head.
Mantrip going over dip and brakes didn't work. Neither did the dead man. Mantrip derailed.
Truck stopped after being loaded and end loader bumped into back of trailer. Truck driver struck forehead on door frame of truck.
SCALE ATTENDANT WAS SWEEPING THE OFFICE FLOOR. FOOT CAUGHT THE BASE/WHEEL OF A DESK CHAIR RESULTING IN A FALL AND SUBSEQUENT INJURY TO THE LEFT UPPER LEG OR HIP.
Employee was cutting a suction hose with a utility knife when the knife slipped and cut EE's left hand.
Employee was climbing up in the cab of Excavator when EE twisted knee stepping on the track.
A Bailey Prep Plant Employee was filling a reservoir for the pumps at the load out facility, when Employee stepped down in between two pallets and twisted right knee. Employee became a lost time on 11/22/2019.
Employee alleges back began hurting while operating a 988 Cat Front end-loader. However we are disputing this claim due to the employee has been seeing a physician for back pain at least 2 years before starting employment with us.
At the 23w room at laydown, employee was unloading supplies on concrete pad when EE took a step backwards causing EE's left knee to buckle. EE was seen by site medic referred to the ER and further referred to orthopedic where EE was issued restrictions pending the MRI results. EE first missed shift was 4/1/19.
Using a pry bar on DMU rails and hurt shoulder/bicep. EE had surgery on torn bicep on 5/30/2019. EE never missed a day work until surgery date.
Employee was welding wear bars at the primary crusher. At the end of shift ee didn't note any issues, after getting home, ee notice vision issues with left eye. EE went to optometrist and was taken off one day of work.
Injured ee was standing on the 1W Section belt tailpiece anchor supervising the alignment of a shuttle car into position with the tailpiece. EE lost balance & slipped off the support causing right ankle to roll when ee landed onto the mine floor. Employee failed to stand in a stable position. Was wearing required PPE and is experienced. No regulations were violated.
EE exited the road grader to clean off the camera. When walking around machine, EE slipped catching themselves with left arm on the grader. EE felt pain in the shoulder area. EE was taken off work May 1st for surgery.
Rock was stuck between grease truck back tires, mechanic removed rock and a knot formed on inside tire. Employee walked over to examine knot on tire when tire blew resulting in debris blowing in both eyes
4900-210 Move wooden post with slusher, end of post caught rib came back hitting miner in lower right leg. Went to clinic - released to full duty on March 26th. Miner went back in on April 4th - doctor put on lost time till April 12th - elevate leg and have MRI done
Employee reported that EE has been experiencing pain in the groin area for about a week. Nothing was reported to management until 3-26-2019. EE was sent to the company doctor for evaluation. The treatment: no lifting more than 20lb, and a Rx for pain. Our site can not accommodate the restrictions, Thus the injury will result in an LTI until a full release from doctor.
Employee was crossing belt line with tools in hands and jumped from side of the belt line landing on a rock thus twisting ankle
5200 Grizzly - using double jack to break rocks on top of grizzly; felt strong pain in right hand.
Employee was using a rope to drag a piece of pipe when they felt a pain in their right shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. The employee began missing work due to the injury on 04/08/2019.
Employee felt a pain in lower left back and left leg while attaching a hoist hook to a basket. Injury became a lost workday injury on Sept. 1 2020.
Employee removed PPE to wash off face and used a hose that was laying in sludge that had caustic in it.
Employee's foot slipped in the truck cab door way while exiting a 6x6 water truck. EE landed on the ground with weight on ee left foot, resulting in a fracture to left heel.
Employee was working inside kiln on brick rig platform. EE's foot slipped and fell off of the platform. Employee landed on EE's right foot, fracturing the ankle.
EMPLOYEE MIDDLE FINGER ON RIGHT HAND WAS BROKEN WHEN IT WAS CAUGHT UNDER A COUNTER WEIGHT.
A maintenance welder was lying down on side to weld a hose bracket on a 994 loader. The employee felt pain in low back when employee pulled self up after completing the job around 9:00am (March 24th). The employee reported the incident the following shift (March 25th). The employee was placed off work on March 29th after a clinic appointment.
Employee experienced muscle damage (bicep) while helping another employee lift a shovel dipper tooth.
Employee exited the mantrip and started walking to the face when EE stepped off of the concrete surface onto the mine floor and lost balance causing right leg to be caught in under EE's body fracturing the right ankle.
Employee was hanging high line cable, EE felt a burning/tingling in the groin area. The injury resulted in a hernia.
Employee was hanging ventilation curtain from a roof bolter. The employee lost footing and jumped from the bolter. The employee landed on feet, but felt a pain in lower back.
Employee reported experiencing discomfort in a finger on the right hand 10-15 minutes after using a wrench to remove a dowel pin. On 4-23-19 EE saw a specialist and has been off work.
Employee was attempting to step off the crawler tail piece at the Longwall when ee slipped and caught self with left arm causing pain in left shoulder. Employee continues to work but ee started Physical Therapy on 04/09/2019. This is now a NDL accident.
The Employee was re-railing cars in the Metz motor barn, repositioning the air bag under the car when EE felt a pop in the left shoulder. The employee did not violate any laws or company policies and was properly trained.
Employee tripped on a sight string for belt alignment that was hanging off belt structure and partially laying in water and struck right knee on a rock resulting in an abrasion. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
A fully trained miner was struck in the nose with a hook when the hook came free from its anchor point on a supply car. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was climbing a fixed metal ladder approximately 8' to reach an access platform. On about the 3rd or 4th rung the employee felt a ""pop"" in left knee. There was no pain, but the next day it felt sore and swollen. After about a week without improvement the employee sought medical attention. An MRI was able to confirm on 4/5/19 that there was a torn meniscus.
Employee was pressure testing a cooler when a piece of metal came off and hit EE's elbow.
Employee was dismounting a skid steer when ee slipped on the step and turned ankle.
5200 Grizzly - using double jack, something popped in left forearm.
Employee was lifting a bag of sand at the mouth of K-1 on Kuhntown bottom and felt pain in the lower abdominal area. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective equipment.
Tramming along haulage route in coal hauler and hit hole in road. Head hit canopy. Employee started missing work March 25th 2019.
IE was loading supplies on the roof bolter tray when IE slipped on a berm/rock and twisted left foot and fell. IE received a fracture to the lower left leg. Left distal fibula fracture.
Technician stepped off ladder and twisted ankle.
EE was removing a cable bolt from top of roof bolter to preform mechanical work, while removing cable bolt EE inadvertently struck self along the forehead. Cause of this incident was employee concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.
IP reported on 3/20 that IP had been experiencing discomfort in lower back for the last several weeks while performing routine tasks in the lab. IP was seen by provider, prescribed physical therapy and given a list of restrictions. Restrictions will not be accommodated.
The employee was traveling on 9 South Mainline Haulage in #70 7man diesel mantrip. The mantrip struck a broken rail at 146block 9 south haulage, causing the employee to strike multiple parts of body against the mantrip, neck, back, knee, and shoulders. The employee didn't violate any laws or company polices, and was properly trained.
The injured employee and another employee were manually moving a screen. The non-injured employee dropped the end of the screen, causing the screen to fall and squish the injured employee's finger.
EE was cutting the wear strips out of a continuous miner in the surface maintenance shop. In the process of cutting, the wear strip fell unexpectedly and struck right hand. The impact caused a cut and bruising to right hand. EE left hand showed small bruise.
EE was standing on the tracks of the excavator to fuel. EE gave a thumbs up sign to another operator in the area. The fuel truck driver thought the signal was to ee and the fueling process was complete. The fuel truck driver began to pull away while fueling was still in progress. The operator jumped off the tracks (approximately 2 ft). EE fell onto right shoulder.
Minor was moving a 55 gallon drum utilizing a hand dolly designed to do so. Minor slipped and fell onto left side. Minor complained of left shoulder pain and later taken for medical care. This has been first aid with employee being observed and not restricted of any duty. Ultimately on November 4, 2019 employee underwent surgery on left shoulder resulting in loss time from work.
Employees began to lift up a block of refractory with a hoist & choker. The load was not centered below the hoist & it began to swing. One of the employees was in the path of the block. EE attempted to get out of the way. The block struck a crossbeam and continued to swing into lower left leg. EE was diagnosed with a hair line fracture in left leg.
EE was working on the scalping tower installing new chutes when a neighbor began firing an automatic weapon in the direction of the screening tower from a neighboring property. One of the bullets hit the EE in the right, upper thigh. The EE was taken to the hospital, treated and released for THE injuries. EE missed two days of work due to the incident.
Employee was pulling straps off stack of straps, stack rolled and landed on right hand twisting fingers resulting in fracture of little finger and ring finger right hand.
Injured employee dismounted a tractor to open a set manual equipment airlock doors. The tractor rolled forward pinning the IE between the tractor and the door.
2 miners in a scissor lift tacked a plate, went down to retrieve the welding lead, on the way back up at 16 feet the tacks broke the plate fell 4 feet on ee left arm causing a fracture. Plate was 3 inches by 11 and 3/8 inch. Was tacked at an elevation of 20 feet on 200 dryer. New mine site still in commencing at time of injury.
A fully trained miner was operating a roof bolter when a rock fell from the top and struck miner's right hand/wrist. The injury was not the result of any compliance issues nor were any protective guards involved.
The ground had become deeply rutted by the repeated haulage of material in that area. A slight layer of ground water had also covered the area making the ruts difficult to see.
Employee was cutting a tree, a fellow employee cut a tree into the first employees cutting area, striking the employee on the back.
Employee stated EE was crawling on top belt running a shiv rope when the belt slid down between rollers causing EE to fall to the mine floor. Employee sought medical treatment and was diagnosed with a strained back. Employee was given days away from work. Note: Belt was locked & Tag
EE was at pipe rack as fork lift was picking up a piece of steel pipe to put onto the pipe rack when the pipe rolled on the ground and onto their foot crushing the foot
Employee stated ee was attempting to shove the feeder shaft from the flat car to the end loader bucket by using knee when ee felt a pain in knee.
Employee was welding in the cement cooler. A piece of refractory broke from the wall, fell about 15 ft. and struck the employee on the hard hat, neck and shoulder. The employee suffered a broken scapula, neck sprain and contusions,
While fabricating a metal rack, employee tipped rack with a forklift to paint all sides. The rack was not secured rolled forward and struck employee knocking arm to ground resulting in a broken arm
Employee was welding in the cement cooler. A piece of refractory broke from the wall, fell about 15 ft. and struck the employee on the hard hat and neck. The employee suffered a closed head injury and neck sprain.
Employee stated that ee was walking to tool box when ee struck head on a low hanging brow. This caused pain in neck.
Cleaning under the conveyor and a rock fell off and hit right ankle.
Miner was crawling in the #3 entry when they fell and hurt their chest on the proximity pad that was in their upper shirt pocket
EE was leaving the section on a permissible 4 wheel ride. As ee was traveling through a flypad ee struck foot out to move the flypad and foot went through a hole in the flypad. EE was jerked off the ride and left knee was sprained.
Employee was climbing up ladder on 930 Loader to start it, when the ladder broke and ee fell skinning shin below left knee, ee just went to a doctor for this injury 3-25-19.
Dozer operator was asked to come to a different location. When ee arrived at new location ee trammed over a rock and the dozer slammed to the ground causing a whiplash type injury.
Employee was sitting pins in the latch coupling of the push beams. EE swung a 20 lb. sledge hammer to hit pin and it glanced off hitting EE's left ankle.
While pulling a plate off stage loader, winch hook came loose and struck employee in back of neck, causing fracture to vertabra.
While operating roof bolting machine, the miner was installing a T3 channel strap. During installation, the strap came in contact with the drilling steel, causing the strap to spin around, striking the machine operator's hard hat on the left side of their head. There were no witnesses to the incident.
EE was lifting a heavy object when they felt a pop in the right calf. They reported the incident to a supervisor and worked out the shift. EE was seen the next day at the clinic, given crutches, and put off work.
Employee was walking and stepped on a piece of coal injuring foot
The employee was working from a work platform, EE was trying to repair a hydraulic leak on a water truck. Another employee was moving the oil cart and struck the bottom of the work platform causing the employee on the work platform to fall.
Employee was changing a screen panel in the plant when employee slipped and twisted an ankle, causing a fracture.
Employee was carrying a roll of fiber optic cable to a worksite. While walking on uneven ground employee twisted knee. Employee worked with a knee brace and no restrictions until surgery on 5-30-2019.
Haul truck operator experienced neck pain from a possible loading event at the beginning of the shift and reported neck pain to supervision at the end of the shift. Shovel operator states a rock rolled out of the shovel dipper striking the truck bed. Haul truck operator states feeling a warm sensation in the side of the neck, and some stiffness by the end of the shift.
Employee was walking between cross cuts #16 and #17 on west main #2 belt line when coal rolled out from the rib hitting employee on the left ankle.
Employee had gotten off grease truck, was walking and step in a small dip in ground and turned left ankle. Just found out ee has been off from work for this injury.
Stacking continuous miner cable. When EE bent over and picked up cable EE twisted EE's back.
Employee stated was drilling a rope bolt hole and was pulling the starter from the hole. The last piece of steel the employee pulled by jerking on the steel and felt a sharp pain in right shoulder.
Employee getting into water truck, slipped, caught right arm.
Employee stated that EE was operating a 12/27 C/M Machine on the Unit 1 Section. EE had just got done cutting the #8 face and was going to back the machine out of that cut. In the process of bending over to grab the miner rope to hook up the cable to the machine the employees back popped and EE fell to the ground.
Employee alleges was prying a rock from a gearbox and it hit employee in right eye and nose knocking off the safety glasses and causing right eye and nose to be swollen.
After completing a work place inspection an employee was in the process of loading back-stope blast holes along with partner using a pneumatic powder loader & while inserting the static free hose into a blast hole a rock contacted employee's hardhat causing lacerations, concussion, and a neurological injury.
EE was attempting to move the tongue of the kiln ramp when ee felt discomfort in shoulder and back. The incident resulted in a torn bicep and rotator cuff.
Employee slipped and fell while walking. Injury to left side.
Employee was closing car doors on train cars using a pry bar when the pry bar slipped causing them to fall
EE was operating the miner in the #5 entry. EE was cutting #5 to #6 break. A rock measuring 3'x4'x2"" to 10"" thick fell from the corner striking EE on the back. EE was knocked down to the mine floor and pinned with the remote box underneath EE.
Boss was helping miner operator to hang cable. The injured person said their back started hurting.
Pulled into #4 Right Return side of the belt feeder dropped bucket, turned off scoop, put hands on canopy to pull himself out of operators compartment and scoop rolled mashing left hand against canopy and mine roof.
Slide gate was removed from an incline auger during operation. The slide gate was not bolted closed at the time of the incident. Lockout/tagout was not followed prior to removal. As they pulled the slide gate open, the fingers of their right hand contacted the rotating auger. This caused a hand laceration and a finger dislocation on the right hand.
IE was assisting in taking a bad piece of cable out of the slope hoist cable. They were picking the cable up to move the cable over. As the IE raised up from picking cable up, IE felt a pain in the lower to middle back. IE did not go to the Dr until 3/16/19 & Dr could only find a possible strained muscle in IE's back. IE started missing on 3/18/19.
Employee was attempting to place the trolley pole on the wire and felt a pain in left shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. This is a questionable injury.
Employee stated they were leaning over checking oil in a grader when they twisted and strained their back. Employee was taken off work under doctors care.
Employee was shoveling at the seal area. As ee twisted to empty shovel, ee lost footing, twisted knee and fell backwards. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. **Employee began missing work on 4/30/2019**
Employee slipped and fell on ice at queue point, causing a closed fracture of lower leg. Currently on-going.
As A bundle of scrap stone was being set down to be dumped, employee had gotten their right thumb between two pieces of stone.
Troubleshooting multi-voltage drive, trying to diagnose 480volt side. I personally locked out 4160 volt several times. A co-worker with a 600 volt meter attempted to check continuity on 4160 and the meter blew up.
EE EXITED CAB AND WALKED ACROSS TOP OF RADIATOR TO CLEAN RIGHT SIDE VIEW MIRROR. WHILE RETURNING TO CAB EE AGAIN WALKED TOP OF RADIATOR AND FELL APPROXIMATELY 8 FEET TO THE GROUND BREAKING LEFT LEG.
Walking from wash our area to speak to manager about where to take load. EE walked behind service truck. Although backup alarm was working ee ignored the alarm and continued to walk as truck driver continued to backup. No damage to truck. EE fell to ground on left side. The driver was flagged to stop.
The employee was walking around 13 Tail when EE tripped over a crib block striking EE's knee on the concrete pad and catching self with EE's right hand. The employee did not violate any laws or company policies and was properly trained.
Fair weather employee slipped on metal stairs and bruised back.
Employee picked up the miner cable and was moving it into the face of #3 entry and twisted upper body and felt right knee in a strain.
The employee was walking thru a cross cut when employee stepped in a rut and sprained left knee.
A contractor employee was walking and pulling a rope. While pulling the rope EE tripped over a hose fracturing left ankle.
Employee was plastering the return side of a brattice, a piece of drawrock fell striking them in the head. Rock broke into two pieces, measurements - 27"" long x 24"" wide x 5"" thick & 20"" wide x 14"" wide x 3"" thick. Incident occurred along 2nd Northwest Mains between the #2 & #3 entries at spad numbers 5237 & 5250.
Employee was re-changing a piece of high voltage cable when EE felt a sharp pain in the left shoulder.
The employee was on the 7 East Section, EE was hanging waterline and miner cable. When EE lifted the cable into the hanger EE felt a pop in EE's left shoulder. The employee did not violate any laws or company policies, and was properly trained.
Employee was attempting to reposition a trough roller which was located inside a skid steer bucket. When the employee pulled on the roller they felt a strain in the left bicep. First Aid given, x-rays (negative). MRI ordered on 6/13/19 which revealed a slight tear in the bicep tendon. Employee referred to Ortho for further evaluation. No lost time, No restrictions currently.
Employee was pulling screen from equipment when screen slipped and hit employee in right side. Employee heard popping sound. Nothing broken, just bruising.
The employee stated ee was in the process of trying to hang some miner in the number three entry when ee felt a burning sensation in the middle of back. The employee sought medical attention on 3/21/2019 and was taken off work.
EE was helping assemble an excavator. Attempting to attach arm to boom with a steel pin (273.25 lbs.). Pin was held in place with a nylon strap. Pieces were misaligned. The EEs began to remove the pin and pieces shifted, causing the injured EE's finger to be pinched between the pieces, causing soft tissue injury. EE was treated for degloving of the distal phalanx of fifth finger.
When the warehouse attendant was manually placing glass blocks boxes on a wood pallet, other boxes fell and hit ee in the legs.
Employee stated that ee was struck on the right hand with a 1"" air impact wrench.
EE was picking up a 6""X20' section of waterline when EE felt a sharp pain in EE's lower back. EE was released to work on 03/12/2019, EE didn't begin losing time until 04/05/2018.
When traveling to the unit on a golf cart, employee went over a hill and noticed a getman broke down in the road. EE could not stop quick enough and had to swerve to miss it. Employee fell off golf cart and strained left shoulder.
On 03/18, EE reported that EE was having back pain that potentially stemmed from a task EE performed on 03/11. On the evening of 03/11, the employee was using a metal rod to clear debris from a bin level sensor. Employee states pain was minor starting on 03/12 but worsened to the point of reporting on 03/18. Incident reclassified to Recordable on 03/20 after visiting clinic.
IE was dragging 2 buckets through curtain when ee twisted to right and felt sharp pain in center upper abdomen.
An employee was lighting a propane weed burner to heat the under side of a hammer mill. EE must have let the propane run too long before trying to light it. It apparently accumulated gas under the hammer mill flashing when EE lit it. It burned EE's face and hand.
Employee was attempting to straighten out an 8' roof bolt to install on the top. The EE felt a pop in left shoulder. The cause of this incident was improper body mechanics. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.
Employee was lifting a relay bar, which was laying on the hand rails and toe of the shield, with a come-along. The relay bar slid off of the toe of the shield and rolled onto the employees right ankle. EE was diagnosed with a fracture.
Employee was lifting the lid on a gangbox and felt pain to right shoulder. No medical treatment was provided at the time of the incident. Treatment and diagnostics were done weeks later.
IE was lifting an electrical component and felt a pain in IE's right arm. An MRI revealed a tear to the bicep tendon requiring surgery to repair.
EE was setting shield sprag, when the initial cave behind longwall happened. The air pressure knocked EE off EE's feet and EE landed on EE's back on shield toe.
Team member was using a sledge hammer to knock coal free from a hopper when EE tore EE's bicep muscle in EE's right arm.
Cone crusher got plugged up with rocks, employee was getting rocks out of cone crusher, footing slipped and fell over ring of cone, ended up going to Urgent Care.
The employee was standing on the second step of a ladder placing a block on the top row of a stopping when employee felt pain in abdomen.
Employee was unloading material from scoop car onto supply cars, doing so EE tripped resulting in a sprained wrist while catching self. Cause of this incident was lack of EE concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.
Employee stated EE and another employee backed the #420 Roof Bolting Machine out by the No. 7 face after bolting it on the Unit 2 Section. The employee stated EE was going to walk around the machine to check EE's dust box. In the process of walking around the machine EE slipped and fell landing on EE's shoulder resulting in a shoulder sprain.
Injured was cleaning dust box on roof bolter. When EE shut lid a small piece of rock hit EE on the top of EE's left foot.
Employee was cutting a coupler out of green discharge line. The line was wet, employee's gloves were wet. The knife slipped and cut employee's left thumb due to bad hand placement.
Employee was shoveling sand at the end of shift and reported to supervisor that back was sore. EE completed shift and went on 3 days off. Upon returning, employee was fine and operated a loader, until a few hours into shift, EE advised supervisor that back was hurting and ee needed to go home.
EE was on a ladder thawing the tail wheel skirting on the spiral plant. When EE was climbing down, the ladder slipped and EE fell approx. 4 feet landing on EE's right side. EE did not seek medical attention until 03/18/19.
Employee was walking from the hoist house to the south side of the surface shop and slipped and fell down.
The EE was lifting a tool bag off a shelf in the elevator. As EE lifted the bag it dropped pulling EE's right arm down, causing pain. The accident was caused by the EE failing to maintain control of the bag while lifting it. EE began missing work on 3/11/19 due to this injury. The EE was wearing required PPE, has training and experience. No rules or regulations were violated.
Opened hatch door to clean plugged cyclone. Climbed down and cleaned them. Forgot to close hatch door. Came back seconds later and fell through the hole. Note: EE has not lost a day of work yet.
Miner was lifting two 32.5 lb. pipes put of pick up and strain lower back/lumbar. Evaluated return to work with follow up on the 11th, returned to work no restrictions but given prescription for muscle relaxer to take at bed time on the visit the 11th
The employee was rib bolting on the 7 East Miner. EE had put self in a bad location trying to get a strap across the miner. When the miner operator sumped the head in, the employee got EE's left leg pinched between the boom of the miner and the miner frame. The employee was wearing all the appropriate PPE.
EE was walking to talk to a mechanic. They stepped between two mechanic's trucks and slipped on ice. Employee was transported to GVMC where x-rays revealed a fractured patella and tibia.
A fully trained miner was unloading rail when their finger was caught between two rails causing a laceration on the left middle finger. The injury was not the result of any compliance issues nor were any protective guards involved.
While changing an expansion joint on a polisher pump, the expansion joint on the pump behind employee unexpectedly burst spraying liquor onto employee's back.
Employee was carrying a tool box when employee lost footing and dropped the tool box on the outside of right foot. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
Employee was walking down the longwall face carrying a rolled up hose. Their right foot slipped forward and they fell backwards injuring the left ankle as they landed on it. EE did not seek medical attention until March 12, 2019. At this time an X-Ray showed a fractured Tibia.
Employee was kneeling down to get a tool out of a locker, when employee went to stand up, EE felt a pop in right knee.
Employee was checking the shear pin on the K-4 coal mill. They reached for a wrench to leave the area and the coal mill clean out door nuts vibrated loose and the door pinched their right index finger between the door and the rotary feeder housing. This resulted in a minor fracture and a laceration of the right index finger.
Employee was positioning a crusher adjustment ring with a pry bar. The pry bar slipped, and they stepped back to catch balance. Right heel landed on a 6"" transition piece and they felt a twinge in the knee. Employee has been performing regular job until being placed off work by an orthopedic doctor effective 4/25/19 in preparation for upcoming surgery on the knee.
Employee improperly attached the secondary line of fall protection to an unapproved anchor point. Employee slipped while moving ee's primary line to allow ee access to ee's work area, and the improperly secured secondary line did not prevent fall.
Employee was operating an empty rock truck back to the pit via the right contour haul road and met a loaded rock truck travelling in the opposite direction. Employee pulled to the side of the road to let the truck by. The rock truck traveled over an irregularity in the road at a high rate of speed, jarring the employee into the right armrest of the operators seat.
A fully trained miner was installing a pulley on a belt drive when the come-a-long EE was using slipped allowing the pulley to strike EE in the right arm. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was a passenger in a jeep that went off track. EE has a history of back problems. EE complained of lower back pain. EE continued to work until they had a MRI which revealed several degenerative conditions in their back. EE was taken off work on 4/2/2019. This is being reported as an Occupational illness due to repeated trauma.
Employee was walking through an airlock and as the employee closed one of the airlock doors the door suddenly shut with high velocity striking the employee's right wrist. Differences in air pressure across the airlock caused the door to slam shut.
While driving a haul truck, associate ran onto a berm. Driver was shaken by the incident & we decided to take EE to a panel doctor. The doctor wrote EE out of work to ensure the EE was well recovered.
A fully trained miner was stepping on a relay bar when ee foot slipped causing ee to fall and turn ankle and jar ee hip. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was removing snow from screen deck and they slipped and fell into chute. Employee suffered a right MCL sprain and a right tibia fracture.
Employee stated Employee was working at the HR64 plant by the rail blow off point on #2 track. EE was climbing down a 5' stationary ladder to a concrete platform. EE opened a safety gate and walked through and closed the gate. Lost grip and fell about 5' against a railing on the other side of the platform.
Employee caught boot on grating and fell injuring right knee. Employee met with Doctor and was put in brace on March 22, after evaluation on knee. Employee is waiting for surgery.
Operating CAT D10R dozer, pushing a big rock out of pit. Using corner of blade to spin the rock out to get behind it. Blade slipped off rock and slammed right shoulder into back of operator's seat.
Employee slipped on ice while exiting vehicle in parking lot. The employee landed on their elbow. Initially, employee did not report an injury. On 3/22/2019, employee reported to work with note stating they could not return to work for 2 weeks. On 4/15/2019 the injury was determined to be work related.
While closing hatch on top of Sand Truck, Sand Truck lurched forward causing employee to wrench back. Fall Protection was in place and worked properly. Sand Truck driver admitted foot slip off of clutch causing truck to lurch.
Employee was Standing with their back to a burn barrel to warm up and caught their coveralls and coat on fire. Resulting with 2nd and 3rd degree burns on their butt and upper legs.
EE climbed D11 dozer to access operators compartment to start dozer. The door was frozen shut and EE could not get the door opened. EE began pulling on the door and pulled the handle off. EE's momentum caused EE to fall backwards off the dozer and strike back/hip against the push arm. EE was transported to GVMC where x-rays showed a fractured hip.
While removing a rock from the coal chain on a bridge conveyor, the rock fell landing on their right foot causing them to fall and hit their right ear off the side of the bridge, resulting in bruised ankle and stitches in the right ear.
Employee was in the process of changing drum liners at 7-18 hoist when employee was hit in the mouth by a bottom belt roller bracket which was being used to pull the wedge out of the drum. Employee required surgery to remove fractured teeth.
A fully trained miner was a passenger on a rail mounted mantrip when EE claims that EE's neck was jarred. EE worked 3/4/2019 and on 3/5/2019 decided to seek medical treatment. The injury was not the result of any compliance issues nor were any protective guards involved.
The employee was walking from the parking lot to the changehouse prior to the start of shift when the employee fell, fracturing left femur.
The employee was climbing stairs and a toe slipped off a step. EE lost grip on the handrail resulting in hyperextension of the small finger of the right hand causing a fracture of the associated bone in EE's hand.
While injured was walking ee fell to the ground on knees. When ee got up, ee couldn't put pressure on right knee. Injured was put off from work pending an MRI.
Employee was helping load miner head onto low boy underground. Employee was in the process of centering the miner head up on the low boy. Employee reached under the miner head to get a chain and the crib block rolled over and struck employee's hand.
Employee parked the pick-up next to the drill to check on driller. The employee went to open the back door of the truck and slipped and fell to the ground on ice. Employee tried to catch self with left arm to break the fall. Employee requested follow up. Has been prescribed Physical Therapy on 4-16-19.
Employee was on landing cleaning windows of Kawasaki 115Z7 loader. Somehow fell through step opening on railing 7'8"" to the ground. No witnesses. Contusion on right knee. Sprain left hip.
Injured had slipped and fell on 3/29/19 and hurt shoulder, had continued working and on 5/3/19 went for an MRI for shoulder pain. On 5/7/19 injured was placed on restricted duty for thickness tear of the supraspinatus insertion without retraction.
EE grinding washers off deck and 4 inch grinder kicked back, striking EE in the nose
EE was looking for hole to direct backhoe to dig for tunnel hole and fell into the hole.
Employee was working at Ball mill #1 tightening the door on the top. When EE went to turn the bar, EE ""felt a pop and a tear in the groin area"". EE was taken by ambulance to the ER. released ""off work"" until seen by a surgeon. EE's first appointment is on 4-12-19.

Employee was with a crew removing demo material. EE was outside the hole removing buckets that the crew inside was filling up. As EE reached inside to remove a bucket the crew inside shouted at EE to watch out because a chunk of refractory material fell from side wall above door. Due proximity of the falling material, EE was unable to avoid being struck in the left hand.
Employee stated that ee was dismounting dozer at the end of shift. EE stepped on a rock which was laying on the step of the push beam. EE foot slipped off causing ee to fall to the ground and landing on right foot. EE said ee felt knee pop which caused severe pain. Went to doctor next day and is off work.
Employee was attempting to open the drop latch on the doors to the screen. Rocks had wedged in behind the doors putting the latch in a bind. Employee felt pain in their shoulder when trying to lift the latch.
Employee was helping pull cable out of road below the 46 shovel. While pulling a portion, they heard a pop in the right knee. At 4:30 a.m., they requested medical treatment. Employee was taken to the ER, and referred to a specialist.
Employee was working on a plugged discharge line that was plugged between a check valve and 4 to 2 inch reducer when they took the vic apart it struck them in the hand cutting their hand open between the pinky and ring finger.
Employee states ""wind blew the J-204 belt off track. I was placing rock on belt and was trying to replace back to original position when the wind blew the belt with rock back up in my face and knocked me back a few feet."" This became reportable when employee sought medical treatment 4/11/19.
Worker was operating a guillotine stone splitter. While adjusting a stone with worker's hand to be split, the worker pressed the pedal to lower the guillotine down to split the stone, and the machine came down on worker's hand. This resulted in a severe laceration on the top and bottom of the hand, wrist dislocation, and bone fractures.
On 8/6/19, the employee needed left shoulder surgery was deemed as work related by Workers Comp Ins. On 03/27/19, the employee reported pulling/straining EE's left shoulder on 03/25/19 when climbing a ladder seven time to adjust chemical pumps instead of using stairway. No medical attention needed or requested at the time. Date of first day of lost Time 08/19/2019 due to surgery.
EE reaching to stop a piece of drill steel from rolling off steel holder and felt pain in right shoulder. EE was placed on restricted duty following medical treatment on 3/28/2019.
EE was moving a fan shroud on a pallet for an R-65 haul truck when EE felt a pop in EE's left shoulder. EE went to doctor the next day and was diagnosed with a strain. Doctor gave EE a cortisone shot and restricted duty for two weeks. After further review, the cortisone shot seems to have fallen under medical treatment rather than first aid for a strain, making it reportable
While pulling a dent out of a fender, employee felt pain in the upper arm. This resulted in a strained pectoral muscle.
Employee was pushing a rock down the rollers on a rock splitter when they heard a pop in their lower back
Truck driver strained back muscle while rolling rock out of the way.
Trees and brush were being cleared on mine property and burned outdoors. The wind blew a spark to an adjacent location and caught some leaves on fire. Employees immediately got fire extinguishers to extinguish the fire. EE inhaled some smoke that caused some breathing difficulty.
While installing skirt boards on #9 conveyor, the EE decided to lift up on the tongue of a trailer and try to move it instead of using a truck/trailer hitch or forklift. The employee strained a muscle in back and has missed time from work due to the accident, thus making the incident reportable.
The Employee had loaded off at the 17 shovel and felt fine. The employee hit a pothole between the 17 and 41 shovel as the employee was heading toward the 910 dump. The employee felt back pain as a result and was transported to the clinic.
MINER WAS BENDING OVER TO LIFT A PIECE OF LIMESTONE APPROX 30 LBS WHEN EE FELT A SUDDEN SEVER PAIN IN EE's BACK AND SIDE AREA
Employee was standing off to the side while the witness was driving the liner out of place. When the witness struck the liner it broke a piece off striking the injured employee.
Employee was trying to move a tail pulley section that was on the ground when EE felt an abnormal feeling in EE's right bicep.
Employee had set their 17 pound back pack down to get safety supplies, after they got their safety supplies that they needed the employee lifted their back pack and felt a sharp pain in their upper abdomen.
Employee was accessing a track excavator when ee slipped and lost footing. Employee was seen at medical the next day and released for next scheduled shift.(3/21/2019) with no restrictions. Employee went back to medical on 3/21/19 and was released with restrictions making this a reportable injury. We are reporting within the 10 day requirement from 3/21/19
Employee was cutting a large box to throw away. EE was holding the box with foot and using right hand with the box cutter. The box cutter slipped and ee cut left hand on the index finger.
Employee aggravated a previous back injury. Not sure exactly what action caused the flare up.
Employee using a Hammer Jack to break concrete. The Hammer Jack slid off the concrete and the point of the tool fell on their foot behind the steel toe. Employee lifted the Hammer Jack off of their foot and inadvertently activated the trigger, causing the tool to puncture their big toe.
Cutting in the face of 1, sumping in felt something struck EE at the bottom of EE's left eye causing 4 stitches.
Two employees picked up the miner cable and walked it to the inby rib. Injured employee felt a pain in the groin area. Employee started missing work on March 13, 2019 for a hernia repair.
Clay was wet from rain. Walking. Pain to right knee and left wrist from fall.
Employee was working inside Raw Mill 1 and fell after material hit EE's right leg. EE fractured right ankle.
The miner was standing on the #4 shaking table (which was not operating at the time of the accident), and was reaching for a slurry pipe. The miner lost EE's balance and fell to the concrete floor. The miner sustained a serious head injury.
EE stepped onto a 3"" water hose and rolled right ankle. EE was taken by personal vehicle to Occupational Medicine for treatment. They examined EE and diagnosed EE with a sprain.
Miner was bending over using a Hammer & Chisel to trim/open a piece of limestone. As EE was striking the chisel, a piece of the chisel flew into EE's lower leg leaving a FB inside the puncture wound.
Employee mounted a tractor on the surface prior to going underground to start shift. After mounting the tractor, employee thought that the safety latch/gate was closed on the rear of the tractor and went to lean back against it when employee fell out approx. 3ft landing on left side.
The employee was walking and tripped over a rock and fell on right knee. Employee was seen by a Dr. on 10/29/2019 and placed on restricted duty on that date.
2 employees carrying metal box down stairs. When 1st employee was setting EE's end of box down to ground it dropped approx. 3 inches from the ground while the 2nd employee was still holding. The employee still holding the box was jarred due to impact on ground and injured EE's elbow. On 3/11/19 employee was placed on work restrictions per medical assessment.
Employee was exiting warehouse C thru the rear door. Employee slipped and fell on the wet concrete floor that had a small section of algae on the floor. They landed on their left shoulder. Employee got up and continued working. They requested to go to doctor on 3/6/19
EE was pulling a hose near header pipe when EE lost balance and fell injuring left hand. Evaluated at ER and diagnosed with a hand fracture.
The Employee came in and notified us on 3-6-19 that Employee's back was sore from work the previous day from working with liners. Employee was taken to the Clinic, had no restrictions with a follow up for 3-11. Today Employee was put on restrictions for a lumbar strain.
The IE repositioned self on top of a weir which caused the employee/operator to lose visual of one another. As the 326F bucket was lowered, it brushed EE's outside right leg which caused EE's inside right leg to scrape alongside the flange of the weir. IE's inside right leg received a laceration from below the knee to the bottom of EE's calf.
Cleaning Dump truck body and a piece of frozen mud broke free and hit leg.
Removing scrap stone from hydraulic splitter, conveyor started and their glove got caught in conveyor belt and pulled left hand under belt.
EE was getting off a 6' ladder. EE was at the second to the last rung and stepped off thinking EE was at the bottom. EE fell landing on right shoulder and hit left elbow.
Employee was checking display lights at scale house, slipped, fell and hit shoulder on rock. Fracture, top, left shoulder
Employee was attempting to load a draw head from the rock duster up on top of the duster and when lifting the draw head said that EE felt and heard back pop. The draw head weighs between 100 and 150 lbs.
Lifting cable for shovel and drills at a berm height, causing pain in the groin area.
Employee was utilizing an compressed air lance in the crusher area, when a piece of rock struck the lance, pinching left hand. 4 sutures were required to close the wound.
EE carrying 5 gallon bucket of oil, switched hands and felt pain in low back. EE was placed on restricted duty after medical treatment on 4/2/2019.
While adding drill rods, the drill helper sustained a compression type injury to the right big toe when their foot temporarily caught between the drill head carriage and the rod slide.

While two employees were repositioning a classifier drive shaft after it was placed on a repair stand, the drive shaft slipped off one of the stand saddles and one of the employees right middle finger was caught between the shaft bearing guide and a stand saddle causing a fracture to the last bone in that finger.
While positioning a sheet of 5'x13' 1/4"" plate steel to weld, the injured employee became momentarily trapped underneath it when the steel came off of the hook used to lift it into position and fell on top of EE.
Employee claims back and left shoulder pain after being rough loaded by Shovel 6.

Employee was starting to climb a small berm, as they started to step they felt a sharp pain in the left calf.
Employee was entering an emptied compartment of a cement ball grinding mill via a ladder and asked for additional portable lighting. A battery operated rated LED light was ""dropped"" down to them, striking the employee on the tip of the left hand ring finger, causing a fracture to the finger tip.
EE sustained laceration to left thumb while using a razor knife stripping fiber optic cable.
While employee was loading stone block on pallet, the stone slipped in employee's hands and pinched finger between the stone and the pallet.
Employee was stacking drained hoses in a pickup when they felt pain in their back.
EE rolled left ankle and fell onto right side. EE did not go for treatment until 4/4/19 and was diagnosed w/a fractured right rib.
At the end of shift, injured employee was descending the primary crusher stairs when ee felt a pain in lower back. Employee went home and when ee returned for next shift a fellow worker informed the site manager that the injured employee was not doing too well. Employee was taken to an occupational clinic for evaluation and was prescribed medication.
Employee was trying to loosen plug on front axle in order to check the oil level and to pull a sample. The plug was tight, so employee was going to shock it by hitting it with a hammer. The employee missed the plug with the hammer and hit finger instead.
EE was helping move, roll-out, and cut old conveyor belting for disposal. While EE was manually moving some belting EE felt a pop in EE's lower back resulting in a lower back sprain.
Strained back after working from a kneeling position and climbing up and down stairs performing maintenance activities. Was placed on modified duty.
Employee was climbing down a ladder, when EE stepped on a air hose fitting, and rolled EE's ankle. Dr prescribed a support boot and restricted walking.
MECHANIC FINISHED WORK ON HOIST CYLINDER ON TRUCK TURNED TO GO BACK TO SERVICE TRUCK AND TRIPPED ON BUCKET OF SKID LOADER FALLING ACROSS BUCKET AND DRAW BAR. BRUISED LEFT SIDE, RIBS AND BACK.
Employee was working on 3/26/19 in dry plant shoveling out built up sand. Employee woke up the morning of 3/27/19 with a sore back and went to urgent care. The employee was diagnosed with muscle strain and was given one day off by the attending physician, and a return to work with restrictions until follow up appointment on 4/3/2019.
While removing a drill fork from a drill hammer the employee's hand became wedged in between the fork attachment and a stationary object. A slow reversed rotation of the drill hammer caused their hand to become lodged between the two objects in the tight space.
EE was removing a scoop from the charger. EE was trying to flip the charger cable up to get it loose from the side of the batteries. When EE made motion to throw the charger cable EE felt a pain in the groin area. The incident became reportable when the EE went to a specialist on 04/12/2019 and was diagnosed with a hernia.
EE was sitting inside 980H yard loader. EE felt a pain in EE's lower left back when EE twisted to the left to clean the cup holder.
Offloading a 40' auger from a flatbed, EE placed chocks against the auger and removed ties securing it in place, then moved to secure it to the forklift with a come-along winch. When the auger started to roll the EE jumped clear of the auger and upon landing on the ground EE's right knee was twisted and EE's left wrist was broken.
Went into dog house to fill out paperwork. Finished paperwork, went to walk out of trailer and hit head on the low hanging cross beam. Happened in dog house.
IW was using a 36"" pipe wrench to take off a water swivel when the swivel broke free and wrench struck EE's left knee cap.
While adjusting toggle plate on jaw crusher, plate slipped pinching and cutting left hand below thumb. Miner returned to work the next day with modified duties.
Employee was walking across the parking lot from personal vehicle at the start of shift and felt a pop in left calf muscle.
On 3/23 employee was working as the ground person at our safe rack, EE was walking to a truck, and bumped EE's knee on a conduit that goes to our kiosk. Upon stepping back EE stated that EE took a mis-step and felt pain in EE's knee. Employee did not report this incident, we noticed EE limping and questioned EE as what had happened.
While bent over cleaning bottom drill holes (post drilling) injured miner was hit in back with rock that fell from face.
Employee was prying bearing housing off with pry bar on a rotary and was exerting a lot of force when pry bar slipped and they felt their right wrist pop and there was an immediate sharp pain.
Temporary employee was underground, operating the #16 haul truck. EE was backing under the loader when EE received first bucket load of 5th trip of the shift. The load shook the truck more than normal and EE felt neck tighten and pain shot through it.
Employee was replacing a spray bar on the bottom deck of the wash plant and bumped knee on an existing J bolt that was installed on one of the bottom pads.
EE was hosing down the wash plant pad when EE felt a pop in shoulder.
Employee turned to go up stairs and caught toe which in turn twisted knee.
Miner was exiting skid steer, slipped, and jammed thumb into bucket of skid steer.
Employee was removing a hoist cylinder from an articulated haul truck. When re-positioning the sling, the pin was removed too soon causing the cylinder to shift pinning employee's hand between the cylinder and a tire.
An operator slipped while accessing equipment injuring knee.
Employee was air arc welding when some slag fell to the ground unnoticed by the employee. The employee stepped on the hot slag resulting in a sore foot later in the evening.
Employee reported lower back pain after being loaded by a shovel in a haul truck.
The employee tripped on a piece of loose flooring. When they put left foot down to brace themselves, they felt a pain in the calf muscle. EE said they felt ok and continued to work. After five days of work they said it was still sore so I sent them to the doctor to be examined. Examination found a pulled left calf muscle.
Employee was transferring five 50 lb chemical bags from a pallet to a chemical feeder. After the fifth bag employee felt a pain in EE's lower back.
Employee using their hands to attempt to manually adjust a wear plate being installed in a hopper when the employee felt a tear in their middle finger on the right hand.
Employees were pushing and pulling on a pipe wrench to break apart a down hole hammer when the hammer broke free the employees lost balance and stumbled into the other employees left ankle.
Swinging a sledge hammer and heard a pop in EE's lower back followed by pain
Employee was working in shop cutting 3"" angle iron piece. Employee was sliding the unused piece back onto rack when the piece of steel fell from the roller onto employee's foot. Employee received two fractured bones in foot.
While scaling a slab on the left rib, the tip of the bar broke a piece off and the bar shock loaded employees hand.
EE was pulling chocks off the rear counter weight of EE's loader with one hand, allowing the chock to fall to the ground while continuing to hold on to it. Due to the use of improper body mechanics and techniques the EE felt a muscle strain in EE's back.
EE was putting a boom/stick pin in the long reach trackhoe. EE bent over to line up the hole and when EE raised back up, EE felt something ""snap"" in the lower part of EE's back. Ended up just being spasms resulting in restricted duty causing the incident to be reportable.

A truck driver/mine employee was on trailer supervising the loading of the trailer. EE took a few steps backward and fell off the trailer and landed on neck. EE sustained a fracture of the seventh cervical vertebra.
Removing steel throat bushing from Slurry pump housing. Hung up with 1/4"" to go. Used left hand to push the top to free it. Came lose tipped back on the chain hoist supporting it and pinned left elbow on pump housing, simultaneously bending left hand back so fingers touched the top forearm. Diagnosis: Closed acute non-displaced intra-articular radial styloid fracture.
On 3/15/19 an employee was cutting liners from a grizzly feeder with a torch. A piece of slag fell on the employee's pants and into their open top boot burning the top of their foot. The employee self treated over the weekend and went to the doctor the following Monday. The employee was diagnosed with 2nd degree burns and treated with antibiotic cream.
Bent over to tie EE's shoe and lower back started hurting.
EE was walking down the east side staircase from the second floor of the pug mill to the ground floor when at the second to last step EE felt EE's right leg give out and EE fell. EE noted pain in the area of EE's right knee and could not bend it.
Mechanic working in vertical raw mill and had to climb up and down to access separator. Mechanic believes EE strained right arm during climbing. Afterwards EE thought it was just sore and did not report until 3/27/19. Mechanic referred to clinic for eval and was placed on work restrictions, no lifting >5 lbs.
Employee was dislodging frozen material from #1 Bag House when EE bumped right wrist causing a sprain.
Employee was helping feed 5/8"" tripper cable on the shovel when EE felt EE's shoulder lock up. 4 months later EE's doctor diagnosed a small tear in EE's right shoulder which EE had repaired.
EE was removing a radiator cap and hot boiling antifreeze burned right arm.
Employee was opening the truck door when the wind blew it open. It pulled their right arm forward and they felt pain in the right shoulder.
EE stepped on drain in floor and fell injuring right shoulder. EE placed on restricted duty on 4/29/2019. Treatment is ongoing at this time.
The employee was exiting a pickup and tripped and fell out onto the ground. The foot was tangled in wiring fed through the passenger door. After medical review it was determined that the right ankle was fractured and that EE would need to wear a boot for three (3) weeks.
Employee was hooking up a trailer. The employee pulled the tongue of the trailer over to align the pintel when the trailer dropped onto the pintel the tongue smashed the employees finger between the tongue and the block the trailer had been setting on.
Employee was moving a 4"" pipeline when EE felt pain in the lower back.
Employee was roof bolting in the # 6 entry and after they drilled the hole, they were inserting glue in the hole when they were struck by a piece of slate rock that dislodged from the roof.
Employee was leaning on a piece of steel, when the steel broke the jack hammer fell about two feet and landed on employee's right foot. Employee went to Elbert Industrial Medicine for treatment. Injury resulted in the foot being very bruised and swollen. Employee is on light duty work at the quarry until foot is healed.
IE was holding the back up wrench while loosening a bolt on the can setter when EE's wrench slipped causing it to pinch left pinky finger between the wrench and ram on the ASV.
While clearing a choke above the impactor, the material flushed pulling the bar that the employee was using downward rapidly causing a strain in the employees, elbow, shoulder and neck.
Employee was driving Cat 740 haul truck and stated back started to hurt. Employee was diagnosed with a sprain/strained back.
Employee was grinding, using a face shield, employee handed bar to a employee on elevated platform, employee on platform grabs the metal bar and pulls it up when ee lost grip of the metal bar, the bar then fell back towards the employee on the ground hitting head, causing a small laceration requiring 4 staples.
Employee was picking stone off the cross conveyor when another stone fell off the splitter discharge conveyor pinching EE's left ring finger resulting in a laceration.
The vehicle employee was a passenger in got stuck on an icy patch on relatively flat ground. Employee got out to push the vehicle out and while pushing felt a ""pop"" in the right calf. Their calf was in pain and unable to bear their full weight. Medical diagnosis was ""strained calf muscle"".
Employee was pressing pins at clinker drag when ee strained back while handling the press. While the incident occurred on 03/09/2019 it was treated as a first aid because the employee did not want to pursue further treatment at the time. On 05/14/2019 following a visit to a back specialist, employee was prescribed physical therapy to aid in recovery.
2 employees were lifting a job box when one felt some soreness in back following the lift. EE initial reaction was to ice it but was instructed to go to the doctor the next morning when ee informed supervisor it did not improve.
EE hit their head on a pipe line for compressed air on the second floor of the packhouse by the inside lane conveyor belt which jerked EE's head back causing EE's neck to pop. At this time EE felt minor discomfort in neck but continued to work next shift without issue (3/10).
EE had to chip out frozen material in conv. 11's tail pulley. Due to the fact the cover does not come off EE was on the ground working overhead while pushing the electric chisel into material.
The injured employee was stocking plate steel in the storage area in the fabrication building. The employee was using their left arm and hand to support 4 sheets of steel. The sheets of 3/8"" steel that the employee was bracing with their left arm shifted, falling back towards employee's arm causing the employee's elbow to strike the steel plate on the other side of the rack.
MINER WAS CROSSING IN FRONT OF A PARKED TRUCK, SLIPPED AND FELL.
Employee was attempting to remove a chunk of shale from a weigh feeder when the chunk rotated up and pinched the employee's left index finger between it and the top of the weigh feeder box, resulting in a laceration approximately 1"" long. The employee received 5 stitches and one day of light duty.
EE was holding a drill and the drill bit stuck and twisted wrist.
IE. was shoveling clay fines from tailwheel of B-11 belt. IE was working parallel to wall, throwing material in front of EE, so EE was not twisting EE's back, nor was material wet or in large chunks. EE was working at a moderate pace about 15 mins, when EE felt a sharp pain in EE's lower back. Taken to phys., placed on restricted duty, released to full duty 2 days later.
Walking up stairs between f-2 and f-4 foot got caught between grates causing strain on left foot
Employee was inspecting truck. Strained back raising hood.
Operator was hooking up a water hose to the Sandvik DD311 drill, when EE turned to return to the drill after turning the water on, EE tripped over something. EE caught self on outstretched hands, receiving a laceration to the palm of EE's right hand.
While cleaning out ice build up in the socket sealing ring, the associate made contact with one of the edges on it, resulting in a laceration on the back of the wrist through the glove. The laceration resulted in 7 stitches.
Wet Slick clay on boot when contacting the step, slipped, and hit shin on step.
The employee was working on the surface at the east north vent shaft discharge water line to fix joints in the pipe. The Employee strained their lower back while using the pipe jack to clamp together discharge water pipes. The Employee was in a crouched position when twisting and turning the jack handle.
Supervisor checked on progress of a welding project. After speaking with welders for about ten minutes and assuming the metal was cooled, the supervisor placed hand on the metal and received second degree burns to left fingers and palms.
EE sustained welding flash to both eyes while welding crusher seat liners.
An employee pinched a finger between a bearing housing and the pedestal base. Two employees were installing the tail drum for a small-scale belt. The injured employee was positioning a base bolt from underneath the pedestal and had their finger slightly under the load when it was let off causing a laceration of the skin. Received 5 stitches and restricted duty.
The electrician was beginning to board the shovel when ee slipped, landing on back on a chunk of coal. The result was a fractured rib and non-treatable transverse process fractures.
Employee states they rolled ankle after they stepped out of cab on 41 shovel in vicinity of PLC room at top of access ladder.
Employee experienced low back while doing normal job duties this morning. Employee denies a specific injury which caused the back pain stating ""I was just lifting tires like I always do at my job"". Employee reports the pain started in the center of the low back and progressively got worse.
Employee was walking to work site to start connecting a water line to a portable pump. The employee stepped into a uneven surface, causing EE to fall, twisting EE's left knee.
EE was hanging 4 cables along the side of screen feed. EE secured the cables with zip ties. EE hoisted the cables up in EE's left hand and as EE lifted the zip tie EE had just secured failed causing the cables to fall while EE was holding them just above shoulder height. The weight of the cables came down and rolled off of EE's hand towards EE's thumb causing it to bend backwards.

The Miner was removing bars from a cement mold when the screwdriver ee was using slipped and struck left thumb and nail.
Employee was handling a K panel when it slid causing injury to their finger.
Individual was bent over, cutting a piece of metal out from between crusher rolls, when a piece of mud that was stuck to the side of the chute fell, struck the employee, knocking EE into the side of the chute.
An employee stepped up on a handrail to look at a screen and foot slipped. EE then fell and hit left ear on the handrail causing a laceration.
Two mechanics were putting a tire on a rim. As one mechanic would pry a portion of the tire bead over the rim with a tire tool, the other mechanic would pry another portion of the tire bead over the rim. When the second mechanic started prying on the tire bead, the first mechanics tire tool slipped and the end of the tire tool hit the first mechanic in the lower lip.

During demolition of a project, EE was holding a piece of steel and after the tube was cut, they pushed more out on the bottom piece then the top piece which caused the top to come down and hit their hard hat and glasses, causing the glasses to cut their eyebrow.
EE was working on 7 section and was using utility knife and cut right finger. EE saw nurse, did not need stitches but nurse put a butterfly strip on cut.
EE was moving bagged uniforms inside the office when a paper name tag made contact with their left eye. FA provided on-site. EE later sought medical attention and received prescription medication.
Employee was making a cable splice on the #4 S/C when EE's hand slipped which caused the knife to cut the tip of EE's left ring finger requiring sutures. Employee did not miss any scheduled work days due to the injury.
Employee was using a 2 pound hammer to remove a key stock from a bushing on a 400 HP motor that was seized to the shaft. While driving out the key, EE hit EE's thumb with the hammer, causing an injury to EE's left thumb. Employee received 7 stitches on left thumb.
Employee on Night shift was cleaning up and felt saw some dust falling, when ee looked up to see lime fell into eye. First Aid treatment was administered. The following day ee did not improve and was instructed to go to the doctor. The doctor prescribed antibiotics as a precaution.
Employee was cutting on a boom on a Cat 749 Excavator with a torch. The torch hose blew a hole in the line, caught fire and burned the employee on EE's left forearm.

Employee received a laceration while picking up trash from the hoist yard by 3 shaft collar. The material picked up were remnants of brackets that were just removed from the shaft that had jagged edges.
Employee took a step to climb excavator, lost footing, fell and hit right arm; employee came back to work the next day. (misreported as first aid case, MSHA instructed operator to re-submit 7000-1 for this case).

Employee released to go back to work same day with no restrictions.
Employee was attempting to pull a stuck chain from a bundle of cable bolts when the chain unexpectedly came loose and struck employee in the upper lip. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
While changing screening, injured knee, on what EE does not know. Next day it started to hurt and after that it started to swell.
While attempting to raise canopy on drill mask, after tramming up and setting the bolter ATRS to install the last row of bolts, a rock measuring 9""W x 17""L x 0-1"" thick fell from the roof, striking the employee on the right hand at the wrist area, between EE's Kevlar arm sleeve and glove. Causing a Laceration which required 4 sutures to close
Employee was driving a haul truck through a patch of rough road and felt a sharp pain in the lower back. Employee reported the incident, was checked out by emergency services and returned to work.
Removing a fan from a track unit and the wrench slipped, resulting in a cut to the palm of employee's right hand, near thumb area. Employee received five (5) stitches.
EE was installing a plastic sheath guard back onto a chainsaw blade. EE was sliding left hand over the blade when their hand slipped & caught one of the chainsaw teeth. The contact caused the EE to sustain a small cut on the left middle finger/palm. The EE was wearing work gloves at the time of the incident. EE received steri strips x 3.
Employee was taking apart a metal pipe with a sharp edge piece of metal that cut their left hand.
Worker was operating dust truck that hit the road edge and tipped over into the berm. Had mild abrasion to shoulder and a small contusion to the back of the thigh. Complained about elbow pains a few weeks after. On 08/13/19, recommended for physiotherapy as an MRI showed minor tears in tendon around the left elbow, making this a reportable incident.
Employee was installing a roof bolt in the mine roof when EE lacerated right hand on a roof plate that required stitches.
A miner was working in the Longwall Tailgate, removing Backflow ventilation structures. As EE was cutting the ties holding the top rails, EE reached up to cut off the brace and when EE cut the tie, the rail had tension on it, causing it to spring back toward the miner, striking EE in the lip, resulting in EE's teeth being pushed through EE's lip.
Ee was cleaning Geho strainers. After isolation, the strainer housing did not fully depressurize. Ee used a wrench to poke through a layer of mud at the top of the housing, causing the housing to depressurize rapidly, releasing hot slurry that struck the ee on the left side of ee's face, and ee's left wrist. Seen at clinic, diagnosed with 2nd degree burns and released to full duty.
Employee cut left hand between the ring finger and pinky finger. Received (7) stitches.
EE was putting a metal plate back on the cutter head when it slipped and caught EE's finger.
As the Employee started to drill a hole in the roof, a piece of the roof spun off of drill steel and struck ee on the mouth lacerating lower lip.
Employee was opening the front gate and caught finger in the locking mechanism of the gate.
The tunnel belt head box became jammed with rock which was impeding operation. Trying to clear the jam with a pry bar, the workers hand was impinged between the bar and I beam.
Employee was assisting another employee with a tower plug. Employee was unaware that kiln dust had accumulated on face shield. The dust fell into employee's right eye.
Employee stated ee was operating a roof bolting machine on the Unit 2 Section. EE was in the process of installing a roof bolt. In the process of installing the bolt, the pie pan plate came through the head of the bolt striking the employee in the nose.
Employee stated was operating the Joy 14/15 C/M machine on the Unit 1 Section. In the process of operating the machine cleaning up some gob a rock flew off of the C/M head and struck the employee in the mouth. This caused EE's tooth to be chipped. NOTE: This employee didn't seek medical treatment until 4/8/2019 and that is why it is now being reported.
The employee was working on 1WHG Section, helping to hang a tube in the #2 recovery chute. A piece of rock/coal fell out of the mine roof and struck the employee in the mouth. The employee received four stitches in EE's gum. The employee did not violate any laws or company polices and was properly trained.
Electrician was changing out motor for a magnet belt during the day. EE went home and felt swelling in right knee. EE found a small gauge 1/2 inch wire in the knee. After going to the med clinic EE was found to have septic arthritis of the right knee.
Employee was reclaiming belt framing and placing it on trailer, when ee got left middle finger in between two piece of belt framing cutting finger. The laceration required stitches.
While throwing posts off the back of the Cat IT Forklift the employee slipped and fell off of the counterweight. Breaking 3 ribs.
Employee stacking bagged material on a pallet and palletizing the pallet. Employee states low back pain.
Miner was bagging grit material and used left hand to guide a stuck bag up the belt resulting in hand getting pinched between the bag and feed roller. Miner did not report the incident to a supervisor until 5 weeks later.
EE was cutting a rubber hose with a knife, when almost cut thru hose, the hose rolled causing knife to cut the palm of the right hand, requiring sutures.
The employee was cutting tape off of motor leads to the disconnect fan. EE then cut the right thumb to the back side of EE's hand. The cut required stitches.
EE was removing GET from shovel bucket with all appropriate PPE. EE struck lip shroud with a hammer, a piece of metal struck EE's right leg below the knee.
The employee stated EE was placing # 728 mantrip back on the rail at break 99 when EE's pinky and ring finger got pinched between the end of the jack bar and the coal rib causing a laceration. The employee sought medical attention where EE received seven sutures. This incident is being reported at medical only.
Employee struck a large rock with F550 lube truck while driving in the pit resulting in a laceration to EE's lips.
EE was chipping hardened cement off a conduit and a large chunk broke free and hit EE in the mouth. Medical treatment from dentist resulted in extraction of two cracked upper left teeth.
Bolting with jackleg, just about to complete the hole, looked up to see how much was left to drill and a piece of rock from the back came down and struck them in the lower right cheek area resulting in a broken tooth and small laceration on lower lip.
Employee was removing the lip shrouds off the BE spare bucket in Etterville using a 12 pound soft blow hammer. While striking the rear of the lip shroud with the hammer, a piece of shrapnel came off either the hammer of the lip shroud and struck the employee in the right shin area causing a laceration.
Employee was performing maintenance on a dust pump. After replacing a butterfly valve and an auger bearing, the system was activated. The pump wouldn't discharge so the employee pressed the power switch again, the pump discharged sending fly ash out through the sliding hopper gate. Fly ash entered around the employee's safety glasses into eyes.
Employee was holding a plate to replace it on a crusher when another employee contacted EE's thumb with a sledge hammer.
Employee was exiting loader with ladder, right foot already on group, when left foot became caught on bottom step, causing employee to lose balance and fall to ground. Employee tried to catch self and fracturing left wrist.
As the employee was opening a bundle of glue, employee's knife slipped and made a small laceration in right wrist. (3) sutures were required to close the wound. The employee returned to work and completed shift.
An employee was working on removing a bearing cap to inspect the bearing on ""B Elevator"" when EE came into contact with a damaged metal seal on the side of the bearing head shaft. While removing a piece of the damaged metal, a sharp edge cut through EE's heavy duty glove into EE's left pinky finger resulting in 4 stitches.
A bee stung the miner on the forearm: The miner is allergic to bee venom. The miner was supervising operations at the dredge recovery location.
Employee claims that while dismounting a rock truck, when the first foot was on the ground, the right knee twisted funny while bringing the second foot down. Ground was dry and surface was flat. After seeking further medical attention, medical professional prescribed physical therapy with the first session being on 04/15/2019.
Employee was roof bolting and got something in EE's left eye. Went to doctor and was prescribed eye drops.
Employee had EE's right hand resting on EE's roof bolter's glue tray as EE was retracting the boom. A drill steel that was lying in the glue tray stopped moving when it got against a stack of boards. EE's right middle finger was forced against the end of the drill steel causing a laceration and fracture to the finger tip.

Employee was marking up the face in preparation for drilling when a rock fell from the face striking the employee in the head, then brushing down the right side of the employee's body. The heading and face was checked prior to work beginning in the area.
IE was opening a cable type gate and the handle slipped from IE's left hand and struck the top of right hand.
Employee alleges was holding back the middle deck rubber and turned on the hose and it slipped out of employee's hands and hit employee in the mouth breaking front tooth in half.

Injured was carrying metal in the surface shop and while placing it on EE's work area, the metal hit EE in EE's face and caused a laceration that required stitches.
Employee was grinding. Face shield fogged. Stopped grinding. Lifted Face Shield. Accumulated dust on shield fell into eyes of employee.
Employee was cutting the skirting off a conveyor belt during a belt change. Using a utility box cutter ee slipped and received a laceration to left forearm.
While fueling 4117 pump in ramp 60, employee went to grab EE's radio out of 1771 lube truck. While climbing up steps employee slipped and hit EE's left side on steps.
While changing a coupler on a 4 inch Aqua line, some emulsion sprayed into right eye.

Employee was checking edge strips on the blanks, when one fell, cutting on the back of EE's right hand. Employee received 2 stitches.
Employee was changing hydraulic pump on section feeder when pump fell on EE's finger.
The injured employee and a helper were rolling a continuous miner conveyor motor on the ground in order to install the motor. The employee rolled the conveyor motor over on to left ring finger causing a broken bone on the tip of this finger.
While working on the stone box floor of the primary crusher on the lube system an EE and IE had the power go out without notice at 07:45. As they were leaving the lighting was very dim as only the emergency lights were on and IE tripped over a pad eye on the floor welded to a steel plate and fell on IE's left shoulder and the left side of IE's face.
EE was washing the ROM tunnel with hot water to get frozen debris off of floor. When setting hose down the hose ruptured and scalded the right arm, resulting in a minor second degree burn.
Employee was helping change a bucket on a skid steer, overexerted and felt a pain in the left side of their groin. Employee taken to Urgent Care where it was determined a bilateral hernia. Employee released to comfort at normal duties. EE has not missed any work due to injury. Employee went to doctor on 3/21/2019 to schedule surgery.
Pinched finger between two pieces of metal. 10 stitches in finger.
Employee was cleaning out gate on Silo 3 and got cement dust in EE's left eye. Taken to doctor on 3/15/2018 where eye was irrigated, cleaned and prescribed eye drops and eye patch.
Employee was on break in the work trailer. door to trailer was opened and at that time, a gust of wind brought in dust/debris. Some sand got into the employee's eye.
After greasing, employee turned to walk down the cat walk & received a minor cut on the finger. The transfer belt is a fully enclosed structure and employee says they cut it on the outside of the housing. They went to hospital & received 2 stitches, was discharged and returned to finish the shift
Miner was dropping trailing cable slack from the mine roof from a roof bolter. While handling the cable the take up reel engaged and began taking up the cable slack. A cable rope hanger hook caught the miner in the webbing of EE's hand between index finger and thumb resulting in laceration/puncture requiring stitches.

Lying on back holding wrench as a backer. Stood up and got a tiny speck of what we think was metal in left eye. Doctor removed it Monday (3/11/19) afternoon, no restrictions, no medications, no time off. Eye still bothered EE on Wednesday (3/13/19) morning. Went to hospital, doctor did not see anything but prescribed antibiotic. No restrictions, no time off, no follow up.
EE was walking backwards as ee was removing belt structure. EE tripped and fell down with belt structure landing on lower legs causing a laceration to left calf that required four sutures.
Employee was installing bolt and was struck by bolt plate causing injury to elbow.
Employee was tightening a bolt on a wear plate on a 980G Front End Loader. The hammer employee was using to hold the bolt slipped, causing the bolt to strike right middle finger while being spun by an impact wrench. The contact caused a laceration requiring 7 sutures to close. The employee was not wearing gloves at the time of injury.
While helping load a large tank on a trailer to move it to another location, the injured cut EE's thumb on a sharp piece of metal requiring six stitches.
While clearing a chunk of shale, employee struck left thumb against a chute bracket while lancing.

Employee was setting cribs when ee pinched a finger on left hand between a crib and the mine roof, lacerating that finger that required stitches.
A piece of metal flew off of a generator and hit employee in right leg
Employee was re-spooling a cable winch. Employee was using left hand to guide the steel cable onto the spool. EE's left thumb got too close to the spool and was pinched by the cable and spool, causing a tear/laceration to the left thumb.
While Bolting on off side roof bolter drill steel got full of dust. Operator while trying to get dust out of steel, the steel hit EE's right hand that was on Bolter Canopy and smash EE's index finger.
EE was operating a winch unloading hydraulic cylinders off of ride, EE caught finger between the cylinder and the ride, pinching the tip of the right index finger, causing a laceration that EE received sutures for. EE did not miss time as a result of this injury, nor was EE placed on restrictions.
Operator was pulling draw rock off top of miner when rock slipped off machine resulting in a cut to palm of left hand.
Manually pulling on V belts for crusher installation, fingers of employee got pinched between the belt and the sheave.
Employee grabbed ahold of handle on drill station canopy and went to stand up when shoulder dislocated. Employee received medical treatment to have shoulder set back in place.
Loader operator was adjusting material on a pallet when the operator's hand slipped, and the stone fell, pinching operator's finger tip between the two stones.
Employee injured leg by blunt force trauma causing 8 stitches. While attempting to climb onto a flat bed trailer to tie down equipment, the employee struck right shin on the corner of the trailer causing the injury. EE was released to return to work with no restrictions after the stitches were administered. Follow up with physician in 7 days for stitches removal.
EE was installing roof bolts at the face when a piece of rock was dislodged and fell striking EE on the head causing a laceration above the left eye. EE received 3 stitches. The piece of rock was approx. 8""L x 4""W x 1"" Thick. EE was wearing required PPE, has training & experience. No rules or regulations were violated.
Employee was throwing a track switch when ee felt pain in lower back. The employee continues to work but started Physical Therapy on 4/23/2019.
Employee was cutting with a torch on the preheater tower when some hot material fell from where ee was cutting and got caught in shirt sleeve, causing second degree burns to left forearm area.
Additional backup support was needed to break loose a drill stem connection that was very tight. The pipe wrench used as backup support slipped and pushed EE's hand into an expanded metal guard that had an exposed edge. The exposed metal cut through the glove and cut the bottom side of employee's right ring finger.
