Accident type, without injuries

The 8' hoist ""tripped"" during its ascent in the shaft traveling empty. The hoist operator took the hoist out-of-servie per protocol. As the mine was left with one operating hoist, all miners were withdrawn to the surface. The root cause was determined to be a failed circuit control board. The hoist was returned to service after the board was replaced.

Operator was shoving dirt out with a D11T Bull Dozer and noticed smoke coming from engine compartment, EE jumped out and used the fire extinguisher on the fire but it was out of control so EE got off the machine and walked away.

A roof fall was discovered in the intersection of No. 5 intake track entry at spad #3519. The fall was 20' in length, 20' in width and approximately 7' to 8' in height. No employees were injured. No equipment or ventilation controls were affected. This area was timbered off at all approaches. The lifeline was re-routed and management reviewed the changes with all employees.

A fall was discovered by EE on the evening shift on 07/30/2019. The fall was located at brk 28 #6 entry 100' inby spad 1231 on Beltway mains. The fall was cribbed and dangered off called into the hot line. The fall was 0 to 2' thick 14' wide and 40' long.

The primary counterweight guide shoes became worn due to the excessive racking of the secondary counterweight, which caused the primary counterweight to become dislodged. There was no entrapment or injuries involved with the incident. It did not involve any compliance issues.

Installed a new skip and had clearance problems and hung up. Unable to bring workers up using the main hoist. Pulled workers out using the emergency escape shaft.

While beginning test trips for 2nd shift, the hoist encoder reset the cage location to 0 while the cage was near the bottom of the shaft. The encoder was found to be working properly once reset.

A Fall occurred in 1st Right - Crosscut 10 between entries #5 & #6. It is approximately 18' wide, 60' long with 2' to 7' of top falling. No one was injured or entrapped. No Ventilation was interrupted. The Fall was Dangered-Off and timbers were installed to prevent access. The Roof Bolts anchored below a 1' thick rock band that appears to have failed due to water saturation.

An unplanned roof fall occurred in the left return between survey spad numbers 339 & 345.

Miners were being withdrawn from the mine after a hoisting shaft was taken out of service. A door on the bottom car of the skip vibrated open during its descent and the door jammed in the shaft. The hoist operator returned the skip to the surface. The other shaft was returned to service and the remaining underground miners traveled to the surface safely.

There was not a specific incident and no structural fires. Buckskin had several coal smokers in the pit that were causing smoke to hang over the west portion of the mine and then drift toward some homes located west of the mine. Due to the smell of the smoking coal a home owner called in to MSHA.

The north skip of the friction hoist over traveled beyond its set point at the surface hitting the overhead protection in the headframe. The skip was taken out of service until the extent of damage could be ascertained, root causes identified, and corrective actions implemented.

An estimated 30T FOG occurred within the X Vein 625 level, at the intersection of the X Vein ore drive and access to the North Zone orebody; a three way intersection. Believed to have occurred at the 0600 firing time; a long-hole stope firing on the X Vein, from the 695 down to the 625, a little over 500 ft away. The 625 is chained off with a No Entry sign, barricade and berm.

Roof fall approximately 94 feet inby spad #12854 on our abandoned North Mains C/M section. Measures approximately 20 feet long by 15 feet wide by 4 1/2 feet high. Area was supported with 5 foot resin grouted bolts and 10 foot cable bolts. Strata is laminated sandy shale.

The 1A capsule at the 1A Shaft was out of service due to a malfunction with the PLC processor. The PLC processor had internal switches that became non-operational, the PLC processor was replaced and the capsule returned to service.

The secondary counterweight became dislodged and twisted the guide rail. There was no entrapment or injuries involved with the incident. It also did not involve any compliance issues.

Smoke was discovered in the 6W coal pit at the top edge of the coal face. The smoke was not extinguished in 30 minutes. No miners or mine equipment were at risk during this event.

#3 hoist unexpectedly stopped. The solenoid valve for the West pinion brake lost power causing brakes to engage. An electrical short in the power connection to the solenoid valve caused the failure.

A lightning strike 1/2 mile from Emigrant CIC plant initiated a small wild land fire. This location is within the POO boundary of the Emigrant mine site. As this fire was small, the location was quite inaccessible and before the MERT members that had been dispatched to Emigrant for an unrelated event could access the fire an air attack plane extinguished the fire.

An unintentional roof fall occurred in 6A miner section 2 Wall #4 Entry on the solid side of the intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions and high humidity.

A inby corner rolled out and was reported in the exam books on the G1 longwall #10 crosscut on 7/21/2019. This did not impede passage or impair ventilation, therefore it is not reportable. This is being submitted to abate citation 9200585. No other 50.20(i) criteria applies.

ROOF FALL: There was a roof fall on the North A Belt at crosscut 5 which was 16 feet wide, 1 foot to 3 feet high, and 25 feet long on the belt.

At approximately 4:05 p.m. a small brush/grass fire was ignited by lightning. There was no mining activity underway near the point of ignition. This fire is being documented with a 7000-1 report as it was not fully extinguished within 30 minutes of discovery and MSHA was contacted immediately.

At approximately 12:50pm on 7/19/19 we had an ignition in the face of #4 entry on the 20E CM section. No one was injured & no damage occurred from this incident. MSHA was immediately called and the incident has been investigated. We submitted a vent plan revision & it is approved to try & keep this from happening again.

An unplanned roof fall occurred on the #1 section at spad 16053 bk #9 in the belt entry. The fall measured 40' long, 18' wide, and 5' thick.

Found an unintentional roof fall at cross-cut #44 on the #2 track and connecting crosscut to the belt line. Dimensions of fall were approximately 18' wide by 55' long by 4' thick.

There was an abrupt motion from inside the car caused by one of the miners while traveling down that activated the governor which caused the elevator to stop therefore causing the miners to be trapped. Elevator service co. responded and miners were released within 50 min. the elevator was inspected and returned to service.

An unintentional roof fall occurred on North Main xc 59.5, #3 entry.

A continuation fall was discovered by the fireboss. The falls encountered were connected by previous falls in the Old 3 West Mains at the following spad locations, 2059 (#1 entry), (#7 entry-2060) and (#5 entry 2043 to 2061). Timbers were set, barriers erected and danger signs hung to prevent employees from entering that area.

An unplanned roof fall was discovered in 15 West #3 Entry at the #4 Crosscut. The fall is approximately 8'high 14' wide and 12' long. All accesses have been dangered off.

A roof fall occurred in the C10 heading between headings M1 and L4. The area was not a working area as it had been barricaded and signed from travel a week earlier due to observed conditions. The fall was approximately 90 ft long, 20 ft wide and up to 15 ft above the roof line at its furthest points. It is estimated that over 500 tons of material fell.

The elevator at BMX portal was out of service due to the brake pump malfunctioning. The brake pump was replaced and the elevator returned to service.

A fall of ground occurred in an advancing footwall lateral heading and it propagated back into the previously excavated round taking out some of the ground support fixtures. The MSHA hotline was called at 9:57 pm to report this incident.

A roof fall has occurred at #6 break on #1 belt line (partial intersection and between block 20' in length x 19' in width and 5' in height). No employees were injured, no equipment was involved. Danger tags were hung and timbers were set at all approaches. No ventilation controls were affected by this incident. This fall will be cleaned up.

The MMU 003 section experienced an unplanned roof fall in the number two heading, at the face two breaks inby spad number 3047. Fell from the coal face approximately 60' to the intersection of crosscut 40. No persons and/or equipment were injured or involved. The fall was 60' long, 20' wide and 7' thick.

The examiner located an unplanned roof fall on the 1 West Submains between spad numbers 659 and 664 between the #6 & 7 entries.

Power dropped from the service provider to the facility. Power was restored to the site but upon testing the service hoist would not function properly. Investigation identified that the soft start on the mg set had failed.

An unplanned roof fall was discovered in the old ML 15 belt at 347 block. The area fell on the inby side of an existing overcast and was approximately 10' high, x 16' wide x 10-15' in length. No one was injured during the fall and the area is dangered off.

Line 2 coal mill caught on fire while operating. Emergency services were called, the mill was isolated and the fire was put out. No injuries occurred during this incident.

An unplanned roof fall was discovered on the mainline haulage between 3-35 and 3-36 crosscut. The fall is approximately 16' wide, 30' long and 10' high. Mine management is in the process of cleaning and re-supporting the area.

Discovery of a FOG in intersection at LD ramp intersection w/ the 1556 level access. During shift a FOG was discovered as a wedge failure dropping approximately 40ST of material from the backs at the intersection to the ground. Several sheared/snapped split set bolts were found in material and backs. The area was closed w/ barricades, berms, & warning signage to prevent access.

At approximately 5:45a.m. a roof fall occurred on No.3 section 006-0 MMU. The roof fall took place in the No.9 entry LOCC intersection approximately 120' in-by survey station 11783. The fall extended from the solid right rib in No.9 into the 8 right crosscut about 15'. The fall measured approximately 35' long X 19' wide X 5 1/2' high.

A roof fall was found at approximately 1:50PM in the intake escapeway between spad number 05247 and spad number 05249 approximately 18'-20' in width by 25'-30' in length by 6'-8' in thickness. The lifeline was rerouted and additional standing supports were added in the area.

An unintentional roof fall has occurred at the XC-12 of the S2H in the #6 Entry. The fall was approximately 15' x 17' X 10'. The fall was barricaded off and a site specific action plan put in place to rehabilitate the roof in the area.

An examiner found a roof fall in the #5 entry. The fall was called into the MSHA hotline at 8:45 pm.

No. 3 Hoist tripped out, would not reset. Called IT in Ontario Canada - long range dial stuck when taken out of man mode - reset manually to muck mode and re-set. Recommissioned shaft - back and running at 11:10 PM.

An unplanned roof fall was discovered at crosscut 12-30 between the belt and neutral. The fall is approximately 16' wide, 20' long and 10' high. All accesses have been dangered off.

EE was using an elevator at 8:10pm when an earthquake occurred in Ridgecrest. Elevator had a tripped relay that was replaced resulting in employee exiting elevator at 9:00pm. Thyssen Krupp was called out to inspect elevator.

A roof fall occurred above the anchor point on the refuse belt under false top. The fall occurred inby the ""S-Turn"" from survey station 26+10 to 26+50. The fall was 40 foot long by 16 foot wide and it varied from 8 to 10 feet high.

A fall of ground was discovered in the LC3830 SD334 on 7.5.19 around 01:00 hours. This most likely occurred at shift change, durning blast, on 7.4.19 at around 19:00 hours, but is unknown. The heading was previously shot at the end of night shift 7.3.19, & was mucked on day shift 7.4.19. From the time the heading was shot until the fall of ground was likely only 12 hours.

An unintentional roof fall has occurred at the M4 Xc-32 in the #4 Entry. The fall was approximately 25' x 20' x 12'.

MD624 had an electrical grid box fire which took more than 30 minutes to extinguish. Truck has been removed and isolated for investigation purposes.

At 1030 am range a pre-shifter found a rock fall on break 23 on 3A belt we called the 800 number and MSHA and State has came out an look at it.

A fall was discovered on 7/2/2019 during the weekly airway examination. The fall was located in the left return at brk 23 #2 entry. The fall occurred in the intersection. The route of travel was not disrupted. Operator built 9 point cribs on all 4 access points and danger off.

An unplanned roof fall was discovered on Main North 2 belt at the 35 block intersection. The fall was approximately 16' wide, 25' long and 6' high. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

An unplanned roof fall was discovered in the ML #4 Belt at 38 block. The fall was approximately 20' in length x 8' in height x the width of the entry. No one was injured during the fall.

It was found that there was a geological feature just above the installed ten foot cable bolts that caused the fall.

Struck by concussion

FMC employees were attempting to seal the hoist shaft at the Bremen Portal at Ken American Paradise Mine No. 9. Steel beams were welded in place metal decking was secured covering the shaft. Methane accumulated in the shaft and caused a series of two explosions 4-5 seconds apart. The second explosion killed FMC employee. No other fatalities or injuries occurred.

EE was traveling up the meadow road to load a truck. While driving up the road, EE shook the bucket to make sure it was clean. The bucket contacted the ground and made the loader suddenly stop and propel EE into the windshield.

Struck by falling object

Workers removing a jaw crusher motor. As the hammer attachment was being positioned towards the crusher motor, the hydraulic hammer attachment fell from the excavator quick coupler. The hammer attachment struck the victim in the head pining them to the work platform.

Over-exertion (Not Elsewhere Classified)

Plant Manager aggravated a pre-existing injury to the right heel tearing a blood blister while walking over a level drill bench. On July 20th, EE died after complications following surgery after an x-ray revealed a broken right heel bone and torn achilles tendon.

Miner was clearing brush with a chainsaw on the outskirts of the quarry. The chainsaw kicked back and cut into the miner's boot past the steel toe causing a laceration on the miner's foot. Surgery was performed three days later resulting in the amputation of one toe on the right foot.

Employee was performing maintenance on a v-belt and sheave assembly when EE's right thumb became entangled between the belt and sheave resulting in the right thumb being severed.

Over-exertion in lifting objects

IE and another employee lifted a ram car ejector cylinder out of the back of maintenance ride #710 and set it on the mine floor. Employee stated EE had pain in groin area. EE continued to work until 7/29/19. EE was diagnosed with an inguinal hernia on 8/5/19.

Over-exertion in lifting objects

Employee stated that while lifting a toolbox off of the battery lid of a rubber tired ride on 07/17/2019, they strained abdominal region. The employee reported this incident on 07/18/2019. The employee was later diagnosed with a hernia and was treated for this issue on 10/1/2019. The employee did not miss any scheduled work and has no restrictions from thier doctor.

Caught in, under or between a moving and a stationary object

Employee was obtaining a strap ready to be thrown across the miner. When employee lifted it up to pivot the strap the miner began to sump back catching employee's right index finger between the strap the employee was getting ready to throw and one of the previously installed straps. Employee was diagnosed with an amputation of the right index finger to the first knuckle.

Employee was in a scoop bucket going to the batteries first and the breaker knocked on the scoop which in turn locked up the park brake striking EE up against the ejector blade.

Caught in, under or between a moving and a stationary object

Employee was helping install a section of dredge pipe when EE got leg pinched between the two sections of pipe. Injury was abrasion/contusion of left lower leg.

2 EMPLOYEES WERE DRILLING WITH A DRILL WHEN ONE EMPLOYEE SLIPPED AND LET GO OF THE DRILL IT SWUNG QUICKLY WITH THE HANDLE STRIKING THE RIGHT JAW. THE RESULT WAS A BROKEN JAW.

Absorption of radiations, caustics, toxic and noxious substances

Employee was cleaning around the 5-Bin Feeder when stung by a Bee.

Ee was descending haul truck and EE's back pack caught, causing EE to lose EE's balance, twisting to the left. EE caught self with EE's right arm. EE was seen at the ER and put off work pending an evaluation with an orhto. Seen by the ortho on 08/09/19 and released to restricted duty, MRI pending.

The employee was using a large bar to knock a large rock out of the cone crusher. The employee was putting a lot of force behind the bar, when the rock suddenly moved, pinching EE's finger between the bar and the side of the cone. Lost time as of 8/2.

Miner stepped off railcar injuring right knee on 7/31/19. EE reported the injury to operator on 8/3/19. EE has remained on full duty, per doctor orders, until they had surgery on 10/1/19 at which time EE was taken off duty until deemed fit to return.

While looking at the spare CFP, employee knelt down to check the sheave and felt knee cap pop out of socket. Employee popped it back into place and continued to work. Later it began to swell. Employee informed supervisor at this time. Employee did not go to the Clinic for treatment until 8/5/2019

Over-exertion in lifting objects

The employee was shoveling at six surface tail when EE twisted wrong and felt ""pop"" in EE's stomach. The employee was taken to CAMC, where EE was diagnosed with a strain to the abdomen. The employee was taken off work. This incident is being reported as an LTA.

Fall onto or against objects

Employee stated EE was attempting to pull the oil feel hose out to put oil in the MRS when the hose came loose employee fell against the plow of the MRS hurting EE's ribs

Crouched down loading lifter holes with anfo. Moving to the left for next hole and right foot slipped, and left knee hit rock lying on ground before putting knee on ground

Absorption of radiations, caustics, toxic and noxious substances

On 7-30-19, EE was emptying 2A bin at old plant when dust blew into eye. EE self-treated by flushing eyes with water. (Note: Prior to this, EE experienced weld flash burn to eyes at EE's residence the weekend of July 27-28, 2019.) EE worked remainder of day on 7-30-19, as well as 7-31-19. Still having burning in eye, EE went to ER on evening of 7-31-19 & received treatment.

IE was on a manbus coming around the corner off of EM track onto MN4 track when 2 locomotives with a shield loaded on a trailer lost traction and started sliding outby towards the manbus IE was on. The manbus headed outby but as they entered into the turn, the locomotives and shield on the carrier collided into the manbus.

Swinging hammer. Thinks pulled right shoulder.

Over-exertion in pulling or pushing objects

Employee stated that EE was scooping in the #6 entry and was attempting to pull the line curtain out from under a rock that was approx. 4' x 5' x 0-18"" thick. EE states that when EE pulled the curtain out from under the rock, EE felt pain in the left shoulder.

While slewing miner cable over to the rib the employee felt a pop in the knee.

Struck against a moving object

IE was driving a manbus around the corner from EM track to the MN 4 track. As IE rounded the corner 2 locomotives with a shield on a carrier started sliding outby. As IE tried to drive outby away from the locomotives, IE rounded the corner, the 2 locomotives with the shield on the carrier collided into the manbus IE was driving.

3200-021 RS heading into workplace when employee got stuck in mud and sand; when getting foot unstuck, miner stepped and slipped on track. Right knee ""popped"".

Struck by falling object

Employee had lifted a lid and was looking at the trailing cables when the lid fell striking employee's right hand causing a contusion and swelling to right hand

Over-exertion in lifting objects

Employee stated EE shut c belt off to remove a rock that was fouled in the structure as EE lifted the rock onto the belt EE felt a sharp pain in EE's groining area

Fall onto or against objects

Employee was walking across the hot start line when they tripped on the steps. They attempted to break the fall by grabbing the hand rail with their left hand. When they grabbed the rail it pulled their shoulder out of place.

Caught in, under or between running or meshing objects

Employee was working on changing the belts on separator fan on mill 4. They had locked everything out and tried. As they were removing the belts the fan rotated and smashed the right thumb and tore off the fingernail on the left thumb as well as the skin. It looks like with other equipment running airflow caused the fan to move.

Over-exertion in lifting objects

A fully trained employee was working in the warehouse and moving a jack hammer when the employee felt pain in groin. The injury was not the result of any compliance issues nor were any protective guards involved.

Over-exertion in lifting objects

Employee was pulling apart ventilation tubing in section when they felt a pain in the right shoulder. Employee received surgery 9/20/2019. From doctor's report on 9/27/2019, this injury is reportable.

Fall onto or against objects

The employee was stepping up onto a platform and lost balance. When employee's leg came back down to stabilize self, employee was cut by a stray piece of metal. Lost time as of 8/2.

Struck against a moving object

The Employee was at the end of the rail on the 1 West Tailgate section. EE was in the process of tramming the scoop down the track entry, outby 2blks to load supplies. The scoop tires slid off the rail causing the scoop to slam down on the rail, and strain EE's neck. The employee was complying with all laws, company policies, and was wearing all appropriate PPE.

While moving roof bolter cable, the rib sluffed off striking the employee.

Employee slipped while dismounting Mack Lube Truck after maintenance.

Struck against a moving object

The employee was hauling coal to the preparation plant on the haul road. The truck got out of gear and flipped over onto its passenger side. The employee was not wearing seatbelt.

Employee was walking down the #4 mainline belt, and had stopped to look underneath the belt at 20blk. EE placed hand on the bed rail while looking under the belt, and coal fell out from the mine roof and landed on EE's right ring finger, causing a contusion on the finger and hand. Employee was complying with all laws, company policies and was wearing all appropriate PPE.

Had a bent bolt close to top, tried to straighten it with rotation, the bolt came around and hit employee on left wrist.

Employee entered track motor and was sitting down to operate when EE felt a sharp pain in lower part of left back.

Struck by flying object

A fully trained miner was running the conveyor of EE's shuttle car when a rock bounced out of the conveyor into the operator compartment and struck EE in the head. The injury was not the result of any compliance issues nor were any protective guards involved.

Over-exertion in pulling or pushing objects

Employee was using a 12 inch pipe wrench at cross cut 13 in 7th South to remove a 2 inch pipe nipple from a fire outlet when the pipe wrench slipped and the employee strained their lower back while trying to regain balance.

Technician was installing final drive on the 6020 excavator. While using the hytorq pump and wrench to torque bolts, EE caught thumb between the bolt and the hub, causing laceration to left thumb.

Left Knee was bumped on screen deck bracket. Initial thought was nothing hurt. Later in the evening some swelling and soreness occurring. Visited Doctor and was determines Left Knee cap cracked.

IE was going to change a tail lift cylinder hose on CM#23. They pulled a shuttle car in to block the tail up to keep it from falling but it wasn't high enough for EE to get into the spot. They raised the tail 4 to 6"" higher & EE crawled in. When IE removed the hose, the tail fell to 4 to 6"" & landed on shuttle car side rail & pinned IE under the tail.

Employee was disconnecting an airline from a pump, when the hose came off, spraying degreaser in employee's face. Employee flushed face and eyes, was taken to the ER, and referred to an optometrist, who took employee off until released to full duty 7/30.

Employee was was doing mechanical work, with a transport chain on links with chisel and hammer hand tools when a particle of metal was released that flew and embedded in the lower lip. Medical assistance to the employee was made.

Employee states the accident was on July 22, 2019. However, EE did not miss any work until 9/15/2020. We were told to make a report once EE missed work. EE stated EE was removing the battery off the Cone Crusher and pulled EE's shoulder.

Employee is a mechanic and was working on the 992 loader, repairing a hydraulic hose. EE was pulling on a wrench and felt a pop in EE's back. EE reported the incident at 5:00 PM to the Equipment Manager. Employee went to clinic the next morning where doctor confirmed it was reportable and would result in modified duty. Employee will receive an MRI and is on modified duty.

Miner went to doctors office and said it was fractured and gave EE pain meds.

An MRI was conducted later on and indicated surgery would be necessary. That surgery occurred on September 16, 2019. That medical treatment and subsequent lost time made this injury recordable as of that date.

Employee was rib bolting on the left side of the miner in 5B TG. EE grabbed a roof strap from the top of the miner and flipped it so EE could slide it over to the right side of the miner. EE's right pinky finger got stuck in the tip-out bolt hole when EE flipped the strap. Diagnosed with a fracture to EE's right pinky finger.

Autoclave operator went to field to change over valves on Geho strainers. A technician was using a wrench on a stuck valve when the valve broke free and threw the wrench, which ricocheted and struck the autoclave operator standing nearby in the face. When EE got into position to handle the rope, EE's back was to the cappel & EE was facing the landing. EE placed their right hand under the head of the bolt to push it up and hold it in place. As the pulled the trigger on the impact, the bolt spun causing a laceration to their right hand. The EE was wearing gloves.

The employee stated they were in the process of cleaning off the top of a spray box when their wrist got caught on a piece of expanded metal guarding, causing a laceration. The employee sought medical treatment where they received three sutures to the wrist. The incident is being reported as medical only.

Employee rigged a 90 degree elbow of a piece of pipe to be removed using a come-a-long. The employee affixed the rigging device by wrapping the chain and hook around the pipe. While trying to remove the pipe, the pipe shifted, and pinched the employee's finger between the pipe and the chain. Employee sustained a fracture and laceration to little finger of right hand.

Miner broke hand while dismounting a loader tire with a hydraulic cylinder. The cylinder slipped while pressing off the inside bead of the tire, smashing their right hand against the boom arm. The miner consulted clinic by phone and initially treated with ice and ibuprofen. Company was aware of reportable injury when an x-ray revealed the fracture on 7/31

Employee was grinding loose packing material from a cone crusher and in the process of setting down EE's grinder it caught EE's pants and cut EE's leg as it hadn't come to a complete stop before this happened.

EE was cutting a rock box off of an idle tail piece. As EE was adjusting position in the rock box the flame of the torch went across right hand. Leaving EE with a burn on right hand.

Opening a tool box slide door compartment on service truck box and cut left middle finger.

Miner was operating a splitter when the rock that was split a rock came off and hit miner in the face.

Employee was walking to side controls to drop the ATRS on the roof bolter, when a rock fell from between the outside pins striking and cutting EE's right arm. The laceration required stitches.

Employee was washing off the 5, 6, 7, kilns cooler deck. Fine debris was airborne. Debris got behind their safety glasses into an eye. Employee couldn't remember if they had rubbed the eye.

Employee was standing approximately 10' to 12' feet from the rear crawler as the #65 High Wall Miner was being trammed to the next cut. When a 1"" x 1/4"" piece a metal believed to be from a roller on the crawler struck EE on the left arm requiring 3 stitches.

Employee was crossing over a 36"" pipe in a sitting position and became unbalanced and fell to ground level. Employee experienced pain in left forearm and was treated by Doctor. Treatment include applying a sling to the arm and prescription pain medication.

EMPLOYEE WAS MOVING A BRAIDED STEEL WATERLINE ON THE HAULAGE UNIT. A STRAND OF THE METAL BRAIDING WAS PROTRUDING FROM THE HOSE . WHEN THE EMPLOYEE GRABBED THE HOSE THE STRAND OF WIRE PUNCTURED THE EMPLOYEE'S RIGHT LITTLE FINGER. EMPLOYEE WAS TAKEN TO HOSPITAL ON 7/26 AND GIVEN INTRAVENOUS ANTIBIOTICS DUE TO FINGER BECOMING RED AND SWOLLEN.

Employee was hand dusting rib when the roof bolt operator swung the boom in to install a middle bolt. A loose roof bolt in the tray caught on the bolter light housing and popped out of the tray striking the employee in the forehead causing a laceration. (14) sutures were required to close the wound.

Employee cut left middle fingertip when attempting to cut an airline hose. The injury required 5 sutures. There were no restrictions.

Employee was replacing hoses on the dust control system on the plant and while cutting a hose the utility knife being used slipped and caused a laceration requiring 4 stitches

Employee was exiting a Brookeville mantrip when employee stepped on the rail causing employee to slip and fall. The employee's face area struck the side of the mantrip during the fall causing a laceration requiring 3 sutures.

Struck by flying object

Employee was using a hand held grinder on the seal flange on the # 4 feed end of the Kiln. A lot of material was airborne. Airborne material got into EE's shield and got past the safety glasses and into EE's eye.

Struck by falling object

EE was taking large rocks out of the cone crusher and setting them on the ledge so they can throw them out and one of the rocks rolled off the ledge and came in contact with EE's 3rd/Ring Finger on the right hand.

Employee was cleaning access dryer door and wiped finger along edge. Employee was not wearing gloves and suffered laceration to finger.

Mounting a tail pulley guard, while on the ground, the employee got up and sand & rock got into the right ear.

Employee was using digging bar where rock was lodged in chute at Primary plant. Rock dislodged, hit digging bar and smashed employee's left pinky finger. Employee has to wear finger splint until cleared by Doctor.

Employee states was ""not watching where going"". Tripped stepping into upstairs control room at dry plant. Caught the fall with left hand. Bent left thumb backwards. Dr diagnosed sprain of thumb.

Struck by flying object

Employees changing out corner shroud; employee struck shroud with hammer, felt something hit EE's leg, and noticed it was cut. EE was taken to the ER, where an embedded piece of metal was removed. No sutures.

Struck by flying object

Miner was working around an electrical box, when a dust particle entered left eye, on 7/24/19 it was not reported until next day 7/25/19. Miner was taken to clinic had a corneal abrasion requiring prescription antibiotics, Eye Drops. Returned to full duty no missed day

Employee received a laceration to the face and dislocated left pinky finger when the employee slipped and fell from an elevated roof bolter platform to the mine floor.

Getting down off truck slipped and hit right hand off truck

EE was entering the lift area of the ""Breaker Tower"" at the Prep Plant through a house style steel door. While EE had the door open, the wind began to push the door shut, so the employee tried to stop it, but the door closed onto the tip of EE's right ring finger causing a laceration and a hairline fracture.

Employee was lifting the cover on the hydraulic tank and a finger got caught fracturing the pinky finger.

Struck by falling object

The employee was traveling on the mine travel road when a piece of roof material fell and hit the employee in the head. The employee was believed to be knocked unconscious. With the employee unconscious Part 50 requires immediate notification.

Caught in, under or between running or meshing objects

Cleaning bolt with Bench Grinder. Did not set Bench Grinder to correct clearance measurement before using Bench Grinder. Right Middle finger got sucked in. Fractured tip of right middle finger

Struck by flying object

Employee was plasma cutting on a loader bucket. EE then was needle scaling the slag off when a piece of slag shot off and got under EE's safety glasses.

Employee received burns on their arm while touching a journal inside a Raymond mill.

Struck against a moving object

Mechanic was leaving the scale house on the access road to the quarry. Mechanic stated that EE ""was only going 15mph and thought EE had a blowout and it jerked EE's truck (2003 GMC 1500) EE was driving to the left flipping EE where the truck came to rest on the cab upside down."" Employee received stitches in EE's left eyebrow/temple area.
