An unplanned fall of ground was discovered at Approximately 3:35 am that took out the 5 foot ground fixtures on August 31, 2019 it was located at 1700 West 8000 production mining block. This incident was reported to the MSHA Hotline and a 103K order was issued

An unintentional roof fall occurred in the 7L longwall section. The fall occurred in the belt entry at 39 x-cut. The fall measured approx. 46ft long by 16ft wide by 15 ft high. The fall occurred due to adverse geological conditions in this area. No injury or equipment damage as a result of this incident.
Warm coal in head chute of module #230 combined with prevailing SE wind, high humidity and light rain caused coal to spontaneously combust. Wind drove the flames to the west, down the belt lines, causing all combustible parts to burn and causing heat damage to the metal components.
On August 30th we evacuated the mine due to a malfunction on the #3 cage chairing mechanism. While chairing the cage, one of the dogs went over center and bound in the dog box. There is a brass guide on the back of the dog that bent when the cage was unchaired. We then initiated a controlled evacuation of personnel from the mine via the #2 hoist.
An unplanned roof fall was located in the primary intake escapeway 5 breaks inby the fan at spad No. 1130. This was in the #2 entry and measured approx. 20 ft. long, 16 ft. wide and 6 ft. thick.
An unplanned roof fall was discovered in the Old 5 North Track at 3 blk. The fall was approximately 16' wide, 30' long and 8' high. It was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance.
A roof fall was found in the No. 1 entry extending over to the No.2 Entry ""Continuation of fall found on 06/10/2019"" The fall measured 9'H x 19'W x 70'L. The fall is located 1bk outby survey station 16087 at 19brk on Laurel Branch Submains. The fall has been Dangered off.
Unplanned roof fall discovered reportable at 27 block 6 North haulage. The outby brow fell of an overcast, approximately 15' long, 15' wide and 8' high. The fall was caused by gradual degradation above the anchorage zone from water and moisture infiltration in the presence of clay intrusions.
An unplanned fall of ground occurred located at the 2000 East 3000 A5 west face production heading at approximately 1:30 pm on August 28, 2019. A call was placed with the MSHA 1-800 hotline and the company did receive a 103 K order on this event.
A roof fall was found in F entry of 7 south at 43+90. Fall dimensions 17 feet wide 50 feet long 7 feet high.
An unplanned roof fall occurred in the #6 entry, 18 cc, outby the North Elevator. The fall was approximately 20 feet long, 40 feet wide, and 8 feet high.
A roof fall occurred on the Main North Intake in entry #8 crosscut 12+15. The fall was approximately 7-8' high, 19' wide, and 19' long. There were no injuries and the fall did not affect ventilation.
On 8/28/2019 a Non-Injury Roof Fall occurred at this mine. The roof fall is located in the No. 6 entry at x-cut No. 24 on the 1-South Mains. The MSHA Hotline was notified and MSHA has conducted an investigation of the Roof Fall. The Roof Fall is approximately 18' feet (There are no injuries associated with this Roof Fall).
On 8-28-19 at approximately 12:30 p.m. the service hoist went down due to electrical issues. At approximately 1:00 p.m. the service hoist had been repaired and was back in operation. No one was injured or trapped.
An unplanned roof fall has occurred in the Alma seam, #5 entry, #38 x-cut; fall is approximately 18 ft. wide, 90 ft. long, and 6 ft. high.
An unintentional roof fall occurred at TG-4 xc 68 between #2 and #3 entries.
An Unplanned and Reportable Roof Fall was discovered at #13 Block in the #3 entry on 30 Butt Intake. There were no injuries, no equipment damage and the ventilation system was not disrupted. This occurrence was immediately reported.
A mechanical failure of a drive-line on a Kress coal hauler caused damage to oil lines which started a fire in the rear of the truck. The ""One Call"" was made for the 30 min. fire qualification. The truck operator was not exposed to any injury as EE exited the truck off the front and remained clear. Emergency procedures extinguished the fire with no injuries to any personnel.
An unintentional roof fall was found during the pre-shift exam for the evening shift, the fall was located in the track entry at BRK 42. The fall was approximately 20' in length X 8'-10' in width X 4.5' in thickness. A clean up plan was submitted and approved.
An unplanned roof fall was discovered in the overcast row from Orndoff return shaft to 9M seals. The fall was approximately 16' wide, 10' long and 6' high. It was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance.
An unintentional fall of the mine roof was found at 223 BRK (survey station: 5326) on 08-25-2019 at 08:35 PM. A pre-shift examiner traveling the #4 (track) entry when EE found the fall. The exact time of the fall is unknown. The fall was reported 18' W x 34' L x 8' thick, from the intersection of the #4 entry, continuing over into the crosscut between the #4 and #3 entries.
All evidence indicates that a bird got into a 34,500 kV overhead power line near the West Decker prill silos. The resulting incineration of the bird resulted in approximately 50 acres of native rangeland being burned. Other than 2 power poles being charred, no structures or equipment were near the fire.
Electric motor shorted. Hydraulic line ruptured and sprayed turbo. Fire burned for 47 Minutes. NO injury
An unplanned roof fall has occurred on the Cedar Grove #1 belt at 15 crosscut and is approximately 18' wide, 5' thick, and 130' long.
A fall of the roof occurred on the 2nd SE Mains, #5 entry, crosscuts 38-39 (intersection - pillar line). The fall measured approximately 30ft in length, 20ft in width, 7-15ft height. Area has been dangered off, standing support installed around fall area, we do not plan to clean up the fall at this time.
An unplanned roof fall was discovered near the Harvey Run Shaft at the #1 Borehole. The fall was 16' wide x 8-10' in height. No one was injured during this fall.
An unintentional roof fall occurred in 5B HG miner section 27 Wall #2 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the shale layer in the mine roof.
A roof fall was discovered in an out-of-service belt entry at 8 block intersection of the 6 North #1 Belt entry. The fall is approximately 20 ft. wide x 16 ft. long x 8 ft. high. There were no injuries, impaired ventilation, or equipment damage as a result of this fall. Area has had additional supports installed at the approaches in an effort to prevent continuation.
Gas Compressor station had been down for maintenance. Start up procedure was started. Due to compressor station being a sour gas location the flare stack has to be ignited. The mechanic said that there may have been some liquid in the system that was expelled through the stack causing the fire to start when the stack flared.
A rock fall was found in the North Mains at spad 30+30 in the #8 entry which fell on the High Voltage cable sending power to the #2 Unit.
The highwall above the 1525 portal collapsed, damaging the portal entrance support & mine air heater intake structure. Evacuation was initiated, all personnel removed & accounted for, UG mine tag-in board was cleared. On surface area of the 1525 portal was barricaded & all area personnel removed. All power, propane & water lines associated were isolated & tagged out.
A roof fall occurred on the H-3 long wall head gate area at cross cut 23. The fall measured 25 feet long, 20 feet wide and 5 1/2 feet thick. The fall blocked the walkway but did not interrupt ventilation.
A roof fall occurred on the 2ME travelway entry #4. The fall was approximately 0-8' high, 19' wide, and 60' long. There were no injuries and the fall did not affect ventilation.
An Unplanned and Reportable Roof Fall was discovered on 8/18/19 between 15-16 bk in the tailgate entry (#1 entry 30 butt)for the F-9 Longwall panel. A continuation of above mentioned roof fall occurred on 8/19/19 in the 15 bk 2-1 x-cut and intersection. There were no injuries, no equipment damage and the vent. system was not disrupted. Both occurrences were immediately reported.
The main hoist PLC card failed causing the #9 shaft main hoist to trip out. The hoist was down approximately 4 hours.
The BMX slope hoist was taken out of service when the brakes on the brake car locked up. The brake contactor malfunctioned and was replaced and the hoist went back into service.
The inlet duct of the coal mill had smoldering material that was ignited from a combination of a blaster firing air into the system and coal dust being present in the system. The blaster forced air into the coal and heat causing an explosion. It has also been discovered a fast acting damper which should have closed automatically did not.
The main hoist electrical drive failure caused the #9 main hoist to trip. The employees that were working underground were safely evacuated from the shaft using secondary escape systems.
During our weekly exam, the fire boss discovered a roof fall in the #1 Entry (Return), 4 Head gate, at XC 49.
MSHA was notified at app. 7:27 a.m. that the elevator was out of service. The cause was determined to be a key switch input wire. MSHA was again notified at app. 8:41 a.m. that the elevator had been repaired, tested and put back in service. No miner was on the elevator when this happened.
There was an unplanned roof fall Old 5-54 Supply Roads x-46 spade 9+60.
The skip is equipped with a roll-up door to allow forklifts to load supplies inside the skip. The rollers jumped the track when the door was lowered after supplies were loaded onto the skip. The condition was not detected when the skip was released into the mine. The leading edge of the door caught on shaft structure and was ripped off. The door was damaged beyond repair.
An unplanned roof fall was discovered at 10-37 between the track and belt. The fall does not affect any ventilation or travel ways. The area has been dangered off.
A non-injury roof fall occurred in the left return on the #8 Northwest Mains in the No. 1 entry just outby spad #1726.
There was an electrical failure on our secondary hoist. The contactor in the main panel failed. The hoist was down for aprox. 1 hour.
An unplanned roof fall was discovered in the #1 entry of the old 6 North Mains between 8M and 9M seals. The fall was approximately 16' wide, 30' long and 6' high. No problems with mining machines, PPE, training or compliance.
An unplanned roof fall was discovered in ML #4 Belt at 42 block. The fall was approximately 50' in length x 8-10' in height x width of the entry. No one was injured during the fall.
Production hoist tripped out on a rope slip fault. Tach was replaced and test runs were performed.
EE called for a freight elevator to 4th floor to take tools down. They pushed button then proceeded to go down to 1st floor. Elevator became stuck at the bottom of 3rd floor. EE called control room to have electricians reset elevator. They could not reset it. An elevator company was called to reset the elevator & EE was released from elevator within 35 minutes.
A malfunction of the slope car motor occurred. All workers were withdrawn from the mine. No injuries occurred during this event. The primary and secondary escapeways were both still available. The motor was changed and the car restored to service at approximately 8:00 pm on 8/9/19.
An unplanned roof fall was discovered at 14-25 crosscut between the track and belt. The fall is approximately 10' high, 10' long and 15' wide. All access has been dangered off.
An unplanned roof fall was discovered in the west approach to the Carpenter Intake Shaft. The fall was approximately 18' wide, 16' long and 6' high. The fall was likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.
The #9 shaft Auxiliary Hoist UPS had an output short that was traced back to the master controller causing the system to trip out.
A inby corner rolled out and was reported in the exam books on the G1 longwall #13 crosscut on 8/6/2019. This did not impede passage or impair ventilation, therefore it is not reportable. This is being submitted to abate citation 9200587. No other 50.20(i) criteria applies.
The #9 Service Hoist had a #1 motor tachometer failure that resulted in the loss of the hoist operation. There were no miners underground when this occurred. The Hoist was out of service for approximately 4.5 hours.
An unintentional roof fall was found in the #4 entry BRK 20 on the 1B Belt corridor, the dimensions of the fall where approximately 25' in width by 25' in length x 6-8' in thickness. Miners were withdrawn outby the area, lifeline was re-routed, area was dangered off, and training was conducted.
ROOF FALL f ENTRY AT 11+89 7 SEAM B INTERSECTION. FALL WAS APPROXIMATELY 7 FEET HIGH BY 17 FEET WIDE BY 30 FEET LONG.
The employee was walking off the face at the G1HG longwall section. As employee passed the head gate drive, the previously bolted and meshed rib unexpectedly failed and the material struck employee resulting crushing type injuries.
Was beginning to storm, miner was walking towards cover when EE was struck by lightning.
An employee was in the process of removing a metal dumpster from the bottom floor of our truck unloading building using a hoist. When they removed the dumpster from the building the hand rail was not put back into position and the employee fell into the lifting bay of the hoist. Fall was approximately 37 feet to the bottom level.
A contractor died at the operation underground. The cause of death is unknown at the time of this report. MSHA issued a 103(k) order on the area. The information in this report is all we have at this time.
EE was working to replace a high voltage starter for the slope belt and was in the process of adjusting the mechanical linkage connected to the switch when EE came in contact with electricity. EE was transported via ambulance to CAMC Memorial where EE passed away from the injuries sustained from the incident.
Employee maintaining equipment in mobile fuel truck lost power and brakes moving down a steep grade resulting in over-turned truck.
Employee was repositioning the spring under the shaker when it sprung and pinched EE's right thumb. The tip of the thumb was severed.
Forklift chain came off leaving the mast ""stuck"" in the raised position. Attempted to ""unstick"" chain/mast at which point chain abruptly began to move and mast dropped. Employee tried to move left hand away from chain/mast but it caught tip of fingers.
The mine received a Self Insured Employer Decision on 6/21/2021 from a medical report dated 6/9/21 awarding a 4% permanent partial disability. This is from an incident from 8/3/19, where the injured tripped while walking down steps in the preparation plant.
A contract employee was helping set-up a winder. A second contract employee was in an elevated position when a metal plate slipped from EE's hand. The plate struck the injured employee on the shoulder. The following were not a factor: rules, regs., systems, equipment, PPE. Attitude may have been a factor. Did not lose time until September 4, 2019.
While laying track rails, employee was using a slate bar to flip a rail when the slate bar sprung back and hit EE across the shoulder and collar bone.
IE claims they bent down to pick up a small piece of grating stuck in mud. They felt a pop in their arm. IE was seen by doctor on 9/10/19 after it was reported to safety department, and was diagnosed with a bicep tear.
A haul truck operator suffered a rough load resulting in strained cervical spine ligaments. The employee complained of shoulder, neck and head pain.
Employee stated that employee was attempting to pull a P-40 Pump out of the mud when employee felt a pain in employee's back.
Employee was standing on a ladder, putting DEF fluid in a scraper and the ladder shifted causing EE to twist EE's back
Employee was using a torque wrench in a tight area during tire repair and the socket came off the nut and finger was pinched by wrench. Stitches were needed on finger.
While working to flip a jaw dye at the Granite Plant, EE hopped out of the crusher and was walking on the surrounding catwalk when EE's boot slipped and EE's leg extended forward, causing pain in knee. EE indicated the sole of the boot was in disrepair and caused the incident. EE did not alert supervisor or plant management of the injury.
Employee had a new on-road dump truck tire up against the work bench, employee was bent over to put cap on and tire popped off the rim, hitting the work bench and then came at employee hurting employee by forcing employee to the ground causing broken right forearm, wrist and thumb bones, with exposed bone on wrist. Also broken right tibia in leg with exposed bone.
Pulling curtain out of gob, felt pop in right shoulder.
While traveling in a shuttle car to the feeder, the employee hit a hole in the bottom and injured back. EMPLOYEE DID NOT BEGIN MISSING WORK UNTIL 9/13/2019.
IE was sealing a return stopping. IE had a bucket of sealant slip out of IE's hand and land on the mine floor. When the bucket hit the floor, a watery sealant mixture went into IE's eye.
During the activity of removing the adjustment bolts from the bearing housing, a punch was used to support one side of the bearing, the employee pulled out the support punch and stored energy caught EE's hand between the bearing and adjustment housing.
While dismounting equipment, employee stepped on uneven surface of coal seam, turning and spraining ankle.
Employee was rehanging miner cable and pushed cable up with left hand, tied it with right hand, felt a pop in left shoulder. On 9/16/19 MRI revealed a full thickness tear to the rotator cuff. Employee started missing work on 9/16/19.
Employee had just arrived on west section and was down on knees waiting to begin work, when employee stood up employee's body twisted and employee fell to right side, which resulted in a broken fibula in right leg.
Shuttle car operator said they struck boom of continuous miner with car. The operator reported to foreman some pain in left shoulder. Continued to work, saw a doctor 9/10/2019 to be checked, Doctor determined possible damage in neck shoulder area. Notified on 10/11/2019 operator will be off work awaiting procedure.
IE was struck in right hand by come-along hook while profiling belt structure causing fracture. Miner was trained, in compliance with law and wearing required PPE.
The operator had informed me that while starting the stoker/furnace it had flashed on EE. EE suffered burns on face as well as right forearm. Due to the flash EE had jumped from the upper deck of the stoker and injured foot, deck is approx. 5' off ground. EE had the loader operator who was on shift drive EE to the E.R.
While prying down material, IE fell and sprained right ankle. Miner was trained, in compliance with law and wearing required PPE.
Employee was walking the 10 inch discharge line in #12 sections return when the pipe came apart striking employee with debris and pipe resulting in sutures to arm and leg.
Employee was making a cable splice on the left continuous miner when a piece of rock measuring 65"" Long x 8"" thick x 8"" thick and a piece 28"" wide x 43"" long x 2"" thick became dislodged from the roof and rib striking employee in the upper back and neck.
The injured employee(ee), was walking into the 17-A track spur when EE stepped on loose material, lost EE's balance and stepped into a rut, hyperextending EE's right knee. EE didn't violate any rules or regulations and has proper training.
Injured was running the miner and had a piece of metal strike them in the right forearm. The small fragment of metal has to be surgically removed from the forearm.
taking out a belt scraper when it slipped out of their hand and broke a bone on the left middle finger.
Section down, 9 men left early (1:00-1:15PM). 9 men traveling on 13 man rail ride. Heard noise, stopped and determined that both leaf springs had twisted and caused ride to lower. They proceeded slowly and made regular stops. Ride struck end of rail, sudden stop, an employee's right side hit operator's compartment.
employee was watching chute load railcars - chute was dripping wet fine/material felt something in their eye - thought it was just dust but was irritated - eye was red the next day - when they came in on 8-29-19 eye was swollen - was sent to dr - rock fines were removed from the eye
REPORTABLE ONLY DUE TO SUTURES - Employee was binding down the versa track duck bill when it shifted catching EE's right thumb between the chain and the equipment causing laceration requiring sutures.
Employee was installing a fly board in the #3 Entry on 1 section. Employee stated they twisted their body away from the machine, and when they took a step, they felt pain in the left calf.
A fully trained miner was pushing the pan out on the longwall when a piece of rock fell between the shields and smashed EE's fingers on the headgate drive. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was by the rib pinner controls when the ATRS jacks were put up and a rock came down from in between the roof straps and struck the employee on the head and back of the neck resulting in an abrasion and contusion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
The employee slipped and twisted and felt pain in their knee as EE was pulling on the roof bolter cable. The employee continued to work until 11/20/19 when EE went out for surgery.
Team was attempting to seat the elevator take up sprockets back into the chain. The take up was stuck and the employee attempted to free it by striking it with a sledgehammer which accidently struck the hardened tooth and a piece broke free penetrating their left thigh. Employee was taken to Concentra and redirected to Urgent Care.
A B45 Operator was dumping the presses into the dump truck. The Operator backed the truck under the second press, turned off the truck and in the process of stepping out of the truck twisted an ankle. The truck was parked rather close to the containment curb and the Operator stepped on the curb while exiting.
Employee was descending stairs and foot slipped on stair tread. Employee caught weight on handrail and strained shoulder. Employee was checked by a physician and returned to full duty and has been working without restrictions until today. Today the employee went out for shoulder surgery.
Employee complained of left shoulder pain after shift. Employee had assisted in shoveling during a clean up after train loading was completed.
EE got hurt on 8-26-19 and continued to work until September 4th. 9-5-19 was the day EE went off waiting an MRI.
Employee was welding and a foreign object got into EE's right eye. EE didn't notice it until later that day.
Employee was operating a low-trac when it quit tramming EE was being pulled outside by a diesel scoop when EE hit something jarring EE hurting lower back, butt, left leg.
Employee tripped on wash down hose while walking landing on left shoulder/arm resulting in a sprain. Cause of this incident was lack of awareness. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency was a factor.
Employee stated EE was operating a scoop outby the section in preparation to build a stopping. EE was cleaning the scoop roadway with the scoop. In the process of the employee operating the scoop two roof bolts which were laying on the ground came in to the operator's deck of the scoop and struck the injured employee in the back. This incident turned into an LTA on 8/26/2019.
Miner was attempting to set up a slurry railcar load out. Employee stated when EE released the loading hose from its stored position it swung into EE. A 6"" victaulic clamp that had a bolt protruding, struck EE in the RH inside elbow puncturing the skin and nicking an artery.
While driving an 830e rock truck and being loaded by a 5500 excavator, the excavator was loading a large rock that slipped out of the bucket and hit the bed of truck jarring the employee resulting in pain to neck and back.
Employee was operating a bobcat walk-behind skid steer cleaning up material at the P5 mid drive. The employee turned sideways in the skid steer to observe their surroundings while backing up, when their foot got pinned between the equipment and an I- beam on the structure breaking their ankle.
While making a splice on the Refuse Belt, the employee crossed over the belt and stepped in a hole filled with mud and water resulting in a sprain/strain. Compliance with rules/regs, PPE, mining euip/systems not a factor. Miner proficiency is not a factor.
Injured was attempting to remove a smaller steel pipe from Kiln 4 analyzer pipe. Employee attempted to use water to loosen up lodged pipe when hot water/steam splashed back into their face.
The employee was working on an air pump between 8-9 seal of the Main West Seals. When the employee attempted to move a large rock to free an airline EE felt pain in right shoulder. The employee did not violate and laws or company polices. EE was wearing the appropriate PPE and was properly trained.
EE was standing on temporary 5'x5' grate platform on feed end of L5 Ball Mill. As EE leaned into the mill to apply sealant to joints the grating slipped off the angle iron it was resting on, sliding out from under EE. The EE fell to the level below approximately 7 feet resulting in a closed fracture to left ankle.
Employee injured left shoulder when EE grabbed the railing while exiting a water truck due to worn step grading.

Employee suffered from suspected Heat Exhaustion. At some point after exiting the enclosed cab of the excavator, employee fell. Employee was found conscious but disoriented on the ground. After falling, EE struck EE's head on an unknown object(s). Employee doesn't know how the fall occurred.
Greasing equipment when hose busted injecting fluid into middle of left hand.
EMPLOYEE STARTING UP DENVER MILLS AND SLIPPED ON GRAVEL TURNING RIGHT ANKLE.
Employee was operating a CAT 740B Haul Truck and hit a rut in the road, causing back pain. The injury was not reported to management until 9/9/19.
A fully trained miner was carrying structure when pulled EE's back. The injury was not the result of any compliance issues nor were any protective guards involved.
While driving a ram car on unit 1 the employee bumped their head against the canopy of a ram car. The IE stated that they drove through a hole which caused them to suddenly come up and in contact with the canopy.
While tearing down line curtain at the face, employee's knee gave out causing EE to strain something. Dr. put employee off work starting on 8/26/19 until MRI can be seen.
IE was doing a pre-operational check on the locomotive that EE was going to operate during EE's shift. As EE was stepping forward, EE twisted EE's right knee. The Dr. found no damage to the knee but took EE off of work for 1 week.
Employee was driving an empty haul truck back to the shovel when the employee hit a bump and jarred back. Employee was taken to the ER and taken off work.
IE was stepping off the roof bolter at the end of shift and hyperextended left knee.
Employee was putting metal fence post in hole when employee slipped and left leg went into hole up to employee's hip, bruising leg and hip.
Employee was driving a jeep thru a track switch when the jeep went off track causing the employee to jam EE's neck.
IE was running a continuous miner. IE was cutting extra height for a future feeder setup. IE noticed blood on IE's pant leg and realized IE had been cut.
Employee states they were double booming a six foot glue bolt and while bending roof bolt straight to install it rolled out and caused them to lose balance, hyper extending their left arm causing pain from bicep to shoulder in elbow area.
During workday EE used a sledge hammer, carried boxes of blasting caps and other tasks. When shift was over they were experiencing pain in neck and shoulders. EE said it felt like a pinched nerve. EE did not know which task caused the injury. MRI revealed fracture of C& and C6 spinous process. Sent home with cervical collar. Will see a neurosurgeon. (Edited to fit 384 characters)
Loading out shot rock while picking up large rock it broke in 2 causing the loader to bounce jarring operator.
Employee was guiding scoop operating in an attempt to get a roll out of the belt. While EE was instructing EE to come in, the scoop bucket struck the employee on the right foot.
IE was walking across the floor and began to feel pain in left foot. EE was taken to the ER for evaluation revealing a fracture to the 5th metatarsal. EE does not recall stepping on anything or twisting in any way.
Employee was starting equipment prior to other employees running the equipment. EE had started a D11 dozer and when EE stepped off the platform onto the track, EE's foot slipped off the wet track causing left ankle to roll to the side causing a severe strain.
The miner was using a wrench to disassemble a DTH drill while a colleague was on the control of the machine. The machine was set in motion while the miner had hand close to the wrench (but not on it). The wrench struck the miner on the right hand thumb, pinching it between the tool and the shaft. Miner was taking to hospital for stitches and x-ray. Thumb is broken.
Employee removed bus bar from scoop charger to meter SCR's. When putting bus bars back on, employee dropped the bus bar onto positive and negative leads, causing an electric arc which resulted in the burn injury. The scoop charger was locked and tagged at the power center, but was not unplugged from the scoop battery. Employee is fully trained and was wearing PPE.
While tramming from #9 entry to the feeder, employee ran over a block causing employee to bounce and strike head on the canopy.
Employee was watching and helping their partner scrape dried cemented rook fill off the jamming loader on 8/19/19. At one point they thought they may have got some dust in an eye but felt fine. On 8/20/19 they reported to a supervisor that their left eye was hurting. saw a site medic, referred to ER, were an exam showed a corneal abrasion, restrictions were issued.
IE was adjusting a fan in the surface shop to direct air towards where EE was working to help keep EE cooler. The IE grabbed a hold of the side of the guard which surrounds the fan to swivel it, one of the fan blades came in contact with the guard causing a piece of the fan blade to break off, causing a portion of the fan blade to go through EE's glove into the palm of EE's hand.
Employee was helping make a belt splice and as employee was using a 4 pound sledgehammer to hit the belt knocker to make the splice a piece of metal came off and struck left knee.
Employee stated walking behind haul-ruck, in fuel building felt back pain. Employee seen by responder, taken to hospital. Made no reference to work relatedness. Stated sought chiropractic care previous day. Sept. 5th, its reported employee stated had been performing several tasks that shift; shoveling, walking, placing wheel chocks for haul trucks, pulling fuel hose, greasing etc.
Employee was tramming scoop down slope and bottomed out in a rut. Worked the rest of shift and following week thinking EE was just sore. EE did not report accident until 8/27/2019.
On August 15th, 2019 at approximately 9:20 am two employees were injured when the face area rolled out and struck the two employees. The area that rolled out was located at approximately shields #7 and #6 and the rock measured approximately 96"" L by 32"" W and 20"" T.
Employee tripped over the edge of the sidewalk in front of the mine office while coming back from the food truck and scraped knee.
During operation the bolter slid sideways and came to rest on top of employee's foot.
Employee was welding within the primary crusher when a piece of hot material fell into the top of a boot and burned their ankle and foot. Employee went to the emergency room and was transported to a burn unit. Diagnosed the injury as third degree burns on their right foot. Employee has not returned to work.
The employee was dragging airline in the 8M to 9M aircouse behind another employee. The employee in front was upwind and kicked up dust causing irritations to EE's left eye. It did not include any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
Employee was walking down overcast steps on the 8 South 3 Belt line when EE slipped and fell. EE tried to catch self and hit EE's left hand which resulted in a contusion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Fly rock from a blast flew over stockpile and hit employee in the leg. Rushed to nearest hospital and diagnosed with broken rt. leg.

While employee was un-choking a chute; the employee had a charged air hose slip from their hand, striking them above the left eye brow splitting the skin. Employee was treated in E.R.; four stitches and two RX's.
EE was at the first tire of the rear axle on drivers side. As EE was directly facing the tire, EE bent knees and squatted down to remove the wheel chock. EE reached approximately eight inches directly in front of self with right hand to pick up the wheel chock. EE was unable to stand up. EE felt a sharp pain in the lower back and went down onto EE's knees.
Employee operating shuttle car attempted to get off of shuttle car and was shocked.
A fully trained miner was ducking under a water line when EE felt a pop in EE's hamstring. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was bolting out of the basket, rock fell, hitting hand rail of basket, rock rolled over onto employee's left leg, causing a break in 2 places
EE was cutting weeds when EE stepped back with left leg. EE said felt knee hyperextend and felt a pop. EE was treated on site by First Responder ice applied, but did not want further treatment. On 8/15/2019 EE reported to work and said EE wanted to be taken in.
Employee strained a shoulder when they lost footing while attempting to install a roof bolt. EE continued to work through 3/28/20 at which time the mine was idled. EE was on furlough through 5/4/20 when the mine partially reopened. Due to the Covid 19 Pandemic, EE elected to be self excluded and remain on furlough. EE started missing work on 7/27/20 when the mine reopened.
Employee was removing a top belt roller when the roller slipped and pinched a finger between the roller and a bed rail. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective issues.
Employee was backing out of the warehouse with a bulkbag. They turned the lift toward the north. Another employee was standing beside another lift with their back to first employee. The forklift employee was driving came into contact with the stationary forklift and then glanced off onto Injured employee's right leg causing a contusion.

The employee was standing beside a 2"" pressurized hose when it unexpectedly burst and caused it to whip an strike an ankle. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective equipment.
Employee was loading a skid steer tire in back of pickup and felt a sharp pain in groin area. Employee did not seek medical attention right away. Employee now had surgery on 10/17/2019 for a hernia.
Employee was in the process of installing the extension drill steel needed for drilling an 8ft bolt hole. The extension was coupled with the starter drill steel and the drill head was brought up to insert the extension into the drill head. The steel did not align correctly and kicked out into the injured employees thumb
Due to repetitive reaching (calf raises) to tie 60 inch bulk bags while the bag was in the raised position, an employee received a muscle strain related injury to EE's calf.
Injured was spading a piece of curtain up on the rib and stepped awkwardly on some uneven ground and buckled a knee. Injured was taken for examination and on 8/16/19 an MRI was performed and discovered injured had a torn meniscus.
Employee loading trucks in pit with 990H stated a 6'x2' rock slid appr 13' down muckpile and struck left front corner of the bucket and felt a pain in neck. Clinic placed employee on restricted duty on 8/14/19, currently assigned to operate smaller loader
Employee strained the right side of neck, shoulder and upper arm pulling on rockdust hose along the 4 West #2 Beltline. EE was attempting to remove the hose from the walkway when EE felt pain down the neck, shoulder and right arm. Injured did not violate any rules or regulations and has proper training. Was wearing required PPE.
Operator had just completed inspection. Operator had climbed up the ladder and was lifting the vertical retractable ladder up to its operating position. As the ladder was coming up it unexpectedly bound up causing the resulting back strain in 2002. Was treated as FA, workers comp filed has seen doctors over the past 17 years and I was informed of surgery on 08/13/2019
The injured employee was loading an empty cable spool onto a flat car when employee's right bicep began to burn and became painful. Employee did not violate any rules or regulations and was wearing required PPE.
Employee was driving Haul truck to pit to get loaded when EE hit a pothole in the pit floor. The jarring caused EE have lower back sprain.
Employee was taking bolts out of a cover, when the bolt came loose employee slipped and twisted falling backwards left foot did not move. Resulted in twisting the knee. MRI reveals Near complete to complete ACL tear.
Miner informed supervisor that they had bumped their head on the windshield of the bulldozer and cracked the glass when the dozer lunged forward as a result of the unit going into park and the seat belt not holding. The individual stated that they had a small bump on their head but was not hurt.
EE using 3ft pry bar on fan shroud to align bolt holes. Bar slipped striking EE in left ear.
Ee was working on haul truck. Ee lost footing on some broken concrete & fell, landing on right elbow. Ee was seen at the clinic on 08/12 & diagnosed with a contusion & strain with possible radial head fracture. *EE seen on 9/4/19 repeat xrays showed fracture at the radial head. Follow-up on 9/11/19 with orthopedist confirmed fracture, ee sent to PT, restrictions not accommodated.
Employee stated was loading roof straps into the scoop bucket when employee's foot slipped causing pain to right knee. Employee continued to work until 12-9-19 when employee had surgery.
Employee was moving a pump hose when employee felt something pop in shoulder. Employee was seen by a doctor and returned to work and was able to perform normal duties. Employee worked normal job until 11/2/2020, when employee was scheduled for surgery on 11/3/2020 causing employee to miss work.
Employee was operating the shearer when a shield unexpectedly was advanced. This caused the employee to be pinned between the shield and panline. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
While climbing on berm to plan next job EE lost foot and fell catching themselves with left arm. EE experienced pain and tingling in shoulder, arm and hand. Subsequent MRI shows a torn rotator cuff. EE had surgery to repair on 10-14-19. Was able to perform supervisory duties until the surgery.
There were NO contributing conditions to this injury. Employee's e-cigarette exploded in employee's pocket on its own.
SPLITTING A SLAB OF STONE, SLAB BROKE, FELL ON LEFT HAND CAUSING CRUSHING INJURY.
Employee stated EE was pulling C/M cable in the #7 entry when EE felt a sharp burning pain in EE's lower back. Employee continued to work the remainder of the shift. On EE's way outside, the employee stated the pain got worse. EE went to the bathhouse after getting outside, and then returned to the mine office to report the incident before leaving mine property.
EE was swinging Drill Boom on Fletcher Bolter to install the inside bolt after installing outside bolt. Piece of draw rock struck EE on the back of the right foot while crawling with boom swing.
Employee was shoveling-cleaning the ""tunnel"" at the crusher when they experienced pain in the groin area, employee suffered a hernia/rupture. Employee was released to light duty.
Slipped and fell at same level while splicing a conveyor belt - lacerated left inner thigh on angle iron.
A rock fell from the roof, striking the employee on the middle and upper back causing them to be pushed into the roof bolter canopy. While waiting on the other operator the injured employee inserted their drill steel into the drill head to check for suction. As soon as the steel was removed and was being placed onto the tray a rock fell striking the employee.
Miner was driving Vermeer Saw from pit area, saw was not in use. Somehow, the Vermeer Saw turned on its side just East of the bottom of the ramp that was about to be ascended. The rear axle was separated from the machine in the accident. The Miner was injured in the right hip area. 911 was called and the miner was transported to the hospital by ambulance.
Employee stated that they were attempting to carry empty oil cans from the feeder through the left buggy dump when they stepped in a hole and felt pain in their left knee.
A fully trained miner was working on the 19 West belt when a rock fell from the top and struck their shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

EE was descending haul truck when they slipped and fell approximately 6 feet, landing on the ground. EE was flown to University of Utah hospital and diagnosed with multiple fractured vertebrae. EE underwent surgery on 08/09/19.
Employee was in the bathhouse showering when they tripped on a non-slip mat causing right arm to bend back. Employee was diagnosed with a torn rotator cuff to the right shoulder and had surgery to repair on 10/8/2019.
The employee strained lower back while retrieving belt structure from the old beltline.
Employee was operating a continuous miner when a piece of rock fell out between roof bolts striking employee.

Temporary Employee got dust in left eye. Rinsed at eyewash station, continued to work. Reported to work the next day with eye still irritated. The employee had shoveled up dust into a bucket and upon dumping the bucket into a larger hopper, got dust into the eye. EE was wearing safety glasses but not safety goggles.
Driver was walking back from the main plant office at Ash Grove in Seattle and tripped over cement and the train tracks. Driver fell to the ground. Driver used right hand to stop fall. Fractured right wrist from making contact with the ground.

Employee was positioning a hook when it spun catching EE's finger resulting in a laceration to the finger. Cause of this incident was employee awareness. PPE was a factor, mining equipment was not a factor, compliance with rules and regulations was not a factor, miner proficiency was a factor.
Employee was loading screen wire onto the roof bolter. When employee got the screen wire above EE's head, employee felt a pop in their right shoulder. Employee continued to work until surgery on 01/09/2020.
Injured stated EE had drilled a 3 foot starter hole and was changing steel to a 3 foot finisher when a rock fell from the roof pinching EE's left ring finger against the drill steel.
Employee was lowering down a counter weight when they slipped on spillage. Employee worked up until 10/31/2019...went off on comp on 11/01/2019.
Employee was rib bolting in the 7 West Section when EE was struck by a rock that had fallen from the roof resulting in a sprain/strain to the left knee and a possible torn ligament. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
The employee was climbing down a ladder. Employee did not fall or trip. The employee re-aggravated a personal injury on last step taken onto the ground. Employee described that employee bothered a previous back surgery / injury.
EE was changing a drill bit. When they repositioned the bit, the drill steel came down and pinched their right ring finger. Laceration required stitches, no fracture seen in xray. Employee originally released to full duty then put off work on follow up.
A scaffold was being dismantled in the clinker cooler at the kiln discharge (riser duct). A piece of buildup from the kiln nose fell and struck EE in the back of EE's hard hat. EE climbed down the scaffold and laid on the ground beside the scaffold refusing treatment until the EMT's arrived. EMS took EE to the hospital, and diagnosed with a slight concussion and released.
Employee was hanging the miner cable and water line in a cable handler when the cable handler struck EE in the right eye.
As the employee was nipping a set of batteries to the scoop and piece of loose material fell from the mine roof striking the employee on EE's right hand. X-rays revealed a hairline fracture which should not require any surgery. EE started missing work on 08/06/2019.
Accident was not reported until 8/15/19. This employee said EE developed an infection in EE's hand later due to this small scratch.
The employee was assisting with cleaning up spilled anhydrous lime around the crushing and screening plant. During the course of this process, the anhydrous lime came into contact with the employee's wet skin, causing 1st degree burns. Lost time as of 8/6/2019.
Employee was carrying large clinker ball removed from clinker discharge. When they stepped off of clinker drag they felt a sharp pain in lower back and pain shooting down both legs following with numb feet. Employee sought medical attention and incurred lost time as of 8/20/19
Loader operator was running production loader in the quarry. At the end of the shift, they reported soreness in upper back near right shoulder. Employee was taken to the doctor and given time off away form work.
Injured employee was installing a metal roof strap when drill canopy came into contact with rib loosening the rib causing it to strike EE's back and lower left leg. EE finished the shift but sought medical treatment on 8/5/2019 and was diagnosed with a left fibula fracture and placed off work.

Employee stated was loading the #400 roof bolting machine on the Unit 1 Section. In the process of loading some roof bolts on the bolter the employee felt a pain in lower back
Employee was placing buckets in diesel scoop bucket when EE slipped and fell causing left elbow to come in contact with the bucket stinger. Employee began missing work on 8-5-2019.
EMPLOYEE WAS WORKING ON EQUIPMENT AND DOOR SLAMMED ON EE'S HAND AND CAUSED INJURY.
Employee was walking and tripped over loose coal. EE's left leg gave way on the loose material, and employee fell. AN MRI was done and confirmed Left Quadriceps tendon rupture.
Employee was using a pry bar to move a waterline out of the roadway along a beltline. In doing so EE felt a sharp pain in the groin. EE started missing work on October 1st, 2019 in order to have surgery for a hernia repair.
Stepping out of the loader and rolled ankle on rock.
Employee climbed into the back of a service truck to get a gas can out. When EE was climbing down EE's foot slipped off of the rear step/bumper. EE hit EE's leg on the bumper.
As Employee was lifting a tire to mount onto a truck they felt a pop. Extent of injury is unknown at this time.
While responding to an electrical problem EE heard on the radio, the miner turned to rush toward the plant. EE tripped over a piece of steel pipe at the welding shop and fell to the ground where EE struck EE's head on a piece of steel and scraped EE's upper right arm.
A fully trained miner was walking across a dolly when the miner slipped on edge injuring ankle. The injury was not the result of any compliance issues nor were any protective guards involved.
Ee was changing a breather and walking under a haul truck. Ee hit head on the steering cylinder. Ee complained of immediate head and neck pain. Seen at ER and diagnosed with closed head injury and neck sprain. Put off work for 1 day. Follow-up on 08/05 resulted in return to full duty.
The employee was at the Harvey Run Hoist doing inspections. While the employee was checking the brakes, employee felt a pop in left shoulder. The employee did not violate any laws or company policies and is trained on this job task.
Employees were removing the dredge pump impeller shell. As they were removing the shell, the shell shifted and struck our employee in the stomach area. The employee experienced bruising in the stomach area. No other injuries.
IE was turning a splash guard on the 6th floor of the Concord prep Plant. As EE was pushing the splash guard EE felt a pain in EE's left shoulder. IE missed no work until 8/16/19. After IE visited the Dr on 8/16/19 for EE's shoulder pain, the Dr took EE off of work but found no damage in EE's shoulder. Possibly a strained muscle that needed to heal.
A fully trained miner was taking a draw bar off a motor when EE felt pain in EE's back. The injury was not the result of any compliance issues nor were any protective guards involved.
The employee received a small cut in left hand while making a movement of a tire of the backhoe loader. Employee received 5 stitches at a diagnostic and treatment center. Employee will return to work on August 9, 2019, as notified.
EE repeatedly swung sledge hammer to release dozer tooth pin, causing discomfort in rt shoulder. Rt shoulder impingement syndrome & tendinopathy.
Employee was changing bits on the miner. They struck one that was stuck with a hammer and a shard of metal hit below the left eye. EE was treated at hospital and released. After 3 weeks of blurred vision EE was seen by eye specialist who removed metal from the eye on 9/19/2019. Employee is off work till released by Dr.
Employee was struck in the head with a rock thrown from behind by another employee who did not see EE.
While performing routine daily work assignments of walking, mounting and dismounting equipment, employee states stepping up into a service truck (from ground to rear bumper/truck bed) and felt a pop in right calf. Employee was seen by on site EMT and taken to a hospital for further evaluation.
Employee was chiseling the top off a piece of stone when it tipped over and fell on EE's right foot.
Miner was picking up a drip pan (sitting on 2"" deep mud) to place it under a stacker when they stepped on an unseen rock of unknown size resulting in a ""Jones"" fracture to the left foot.
Employee was climbing onto the 344 CAT drill to check water level and felt a pop in their right elbow.
Employee was using a shovel to load sand onto a conveyor belt during a routine maintenance task. Employee reported to supervisor employee felt discomfort in their back at end of the task.
While attempting to adjust the light on the cab of 990 loader, the employee fell 10 feet fracturing femur.
Employee was kneeling down, when employee stood up to walk employee felt a pop in left knee. Employee has a torn meniscus.
In the course of performing normal duties during a shift, an employee experienced pain in the left shoulder. Employee thinks this may have occurred during a Rotex screen change. Employee visited a doctor two days later, receiving lifting restrictions.
While replacing the right front flat tire on a 2004 crew vehicle, employee strained back and ribs. Employee was offered help from another worker, employee declined help.
While putting air in tire in the shop, the tire sidewall blew out causing the tire to flip over knocking the employee down causing the tire to land on right ankle.
Employee was changing the screen out on a screener deck, which was locked out. The adjacent conveyor was still running, however. As the employee lifted the screen, the screen was pulled into the running conveyor, pinching left index and middle fingers against the running conveyor belt. Employee suffered a laceration and tendon damage to left middle finger.
Employee was manually sliding a vehicle gate shut. While pulling the gate, EE felt a pop in the back side of EE's right knee, followed by pain. Soft tissue injury (strain resulted). Specific diagnosis is pending results of an MRI.
IW hurt shoulder removing windshield from jumbo. Saw PA in Fairbanks 8/27, MRI 8/28 no definitive diagnosis at time. IW flew home 8/28. Orthopedic specialist seen 9/18 due to appt. availability, Physical Therapy x 3wks started 9/23, with definitive paperwork for no work allowed for IW.
While changing screens another employee was handing a screen off to the injured, when it slipped striking injured employee in the elbow injuring the elbow. Employee sought medical treatment and was given light duty work for a week and medication to allow the elbow to heal.
Employee was next to a floor opening while tied off to an adjacent 3/8"" wire rope cable used as a barricade at the opening. EE was pulling on a compressed air hose and lost balance, causing them to fall into the opening. The floor below was only 12' below their location, and their feet still struck the floor below, causing them to twist their left foot.
Operator of the 563HT states they were jostled when a large boulder was placed in the haul truck on the first dipper. Employee was injured on 8/24/2019 and did not report injury until 8/30/2019.

Employee was extracting singles from 4"" pipe using an 18-volt cordless drill and spear. While operating drill employee's hand got wrapped around extracting spear that was attached to drill causing the employee's hand to twist resulting in injured fingers.
Contract Truck Driver was injured when EE fell over tightening the load straps.
Employee was changing screens and a bar dropped in 1/2 bin employee tried to retrieve. Employee fell which resulted in a broken leg.
Was pulling a piece of 6"" thick stone when they felt pain in the left shoulder. Stone was on rollers about 48"" off of the ground.
Employee was driving 5997 on the north side. The truck the employee was driving went through berm onto lower level. Truck rested on the ledge.
EE was removing a bolt from a concete pad. EE was holding a chisel and swung a 4 lb hammer attempting to break the stud bolt. The employee missed and made contact with left thumb, fracturing the thumb. EE was given restrictions.
Employee was preparing to wash spillage from under a conveyor. A new nozzle that was being used was 1.5"" thread and so was the mating threads. The threads were close enough to engage and tighten. However, the unknowing different thread type did not hold pressure resulting in the hose blowing free and striking the employee in the upper lip.
The miner was lifting a piece of steel for one of the conveyors in the quarry. While lifting the piece of steel they felt a pop in right arm/shoulder. The information about this injury was obtained through reports taken by the safety director at the time of the injury. Who was no longer employed at Rogers Ready Mix & Materials as 10/14/2019
Mechanics were using a steel hammer to remove old teeth from a 994 loader bucket. A piece of fly metal came off the hammer and entered the employee's right forearm.
Employee turned ankle while walking to personal vehicle in the Mill parking lot.
Employee climbed the ladder to enter the cab of the crane EE was operating. When EE attempted to step into the cab, EE misstepped and fell to the ground breaking right arm
Employee was changing a screen associated with a small hammermill when EE felt a pull in groin. EE did not report it nor seek medical attention for the injury. On 8/27/19 EE reported EE was diagnosed with a hernia which EE feels was associated with the pull EE felt during the screen change.
The operator was on the Dozer and apparently mooned some other co-workers and fell off the Dozer and injured a knee. I did not find out about it until 08/22/2019
Employee was with spreading hot Alumina to prep it for vacuuming. EE was using a 3"" straight edge rake, to pull the material towards EE to spread it out. As EE did this, the material came towards EE like a wave and ""blew"" at EE causing it to go at EE's leg and run down into EE's boot, causing 2nd and 3rd degree burns to the top of EE's right foot to just above EE's ankle.
Employee was standing unloading supplies off of the #23 mantrip for 2nd shift when the #22 4 man ride pulled forward and pinched the back of EE's left calf between the 2 trips. Employee required sutures in EE's left calf.
The jaw crusher was plugged with rock. Employees were removing smaller rocks by hand. EE lifted a rock and their elbow popped and began hurting.
EE was installing flexcos on a conveyor. EE neglected to ensure the conveyor was locked out. The conveyor started without warning and EE was deposited into a feeder that was not in operation. EE fell 4 feet to the chute on the feeder and rolled/slid into the feeder. The exact severity of injuries is unknown currently.
Employee said they were coming down stairs and turned on platform twisting left knee.
Employee was completing the shut down procedure at the plant in the quarry. With a lot of resistance when shutting the wash-back valve on the dewatering screen, the employee struck the lever with the palm of right hand to shut off the valve.
Holding star frame in the mill with left hand. Reached out to let chain fall down with right hand and when I pulled the chain I felt something pop and it started burning up and down my arm.
The injured was in the process of uncoupling a new Irwin supply car by using the foot operated decoupling device. The foot operated device didn't completely uncouple the car. The injured lifted the decoupling device on the draw head with both hands and felt a pop in the right shoulder causing pain in the right shoulder.
Pulling power line with company truck, line became twisted. EE stepped over line to untwist it, power line recoiled & struck EE in right foot area causing them to fall on right shoulder. Shoulder dislocated, EE corrected dislocation. Treatment by EMT-minor pain, medical dept 8/12, co. dr ordered MRI. Results pending until ortho dr. appt on 9/11. Ordered PT & issued restrictions.
The employee was rolling a piece of steel on a Webb plate bending role in the Maintenance Shop when EE's finger was caught between the plate and roll. The employee's left ring finger was crushed resulting in a laceration and fracture to the end of finger.
While stepping up onto the dredge out of the row boat, employee's knee popped and caused them pain. Employee has a slight tear in the cartilage below the knee.
Employee was palletizing and used their left knee to push on the stack of rocks to straighten them causing a bruise reported on 8/9/19. Employee requested medical attention on 8/12/19.
Injured was roof bolting and was pulling drill steel out of roof and said EE felt a pop in EE's shoulder. Injured was taken for examination and was placed on restricted duty.
EE was performing maintenance while standing on the counterweight of a loader. EE misstepped and fell. EE broke collarbone.
Employee was using clamps to help pull liner into place. Clamps slipped off the liner and employee fell backwards, landing on left hand and injuring wrist.
Employee was moving a pneumatic table and when the air had not fully released employee forcefully pushed it and employee twisted their back when it moved.
Operator slipped and fell to the ground, breaking a wrist.
Right index finger tip pinched between v belt and a sheave during a belt change. The injury was a fracture which needed surgical repair so the employee lost three workdays while getting medical attention. The employee returned to work on 8/12/19.
Ee was replacing a filter cloth when EE bent down to pull the cloth EE reports pain on right side of low back and right shoulder. Seen at clinic, diagnosed with a right trapezius strain and right para spinal lumbar strain. Released to restricted duty.
Operator's left foot slipped and operator fell to catwalk, causing contusion to left shoulder.
EE reported working on stock feeder belt when a pulley slipped contacting EE's hand causing injury. Evaluated in Emergency room.
While employee was tightening a nut with a torque wrench the locking arm slipped causing the torque wrench to jump off the nut jarring the employee's right shoulder. At a later date employee complained of continuing shoulder discomfort. A MRI was performed on 10/15/19 and employee placed on restrictive duty 10/16/19.
Changing a screen, went to come down ladder, miss stepped missing rung, lost grip and fell.
Employee had a slate bar in dilute impeller pulling down when impeller broke loose and employee fell to the left with both feet on the floor. Two other employees were on the other side with slate bar pushing up.
A powder employee was working near a highwall when a rock fell and struck them in the back, causing bruising and pain with breathing.
EE removing a fan from a bulkhead. EE felt pain in neck and upper back from cutting through cables. EE placed on restricted duty on 8/6/2019 after medical treatment.
Employee cut left middle finger holding a bolt with hand while using an impact on the nut attached to it.
Injured stuck EE's right hand into gravel chute to remove stuck rock. Rock shifted smashing middle finger. Co. Dr. splinted finger and returned emp. back to work with limitations.
While putting a belt on a pump sheave, middle finger got caught between the belt and sheave.
Miner was shoveling under a Jaw Crusher when they strained their left wrist and right elbow. Employee was sent for medical evaluation and placed on restricted duty. Miner returned to work and is currently working restricted duty.
Miner injured EE's back while shoveling at the Sullivan Wash Plant. Due to the amount of pain EE is in another miner working with EE is transporting EE to the North Valley Hospital.
Miner was moving a piece of scrap iron and mashed/cut the tip of the 4th finger on EE's right hand.
A miner was injured on the right hand from a socket while using a pneumatic impact gun.
While climbing up the ladder into cable realer 686, employee dislocated right shoulder.
Connector was positioning a motor base for rigging to be removed. Connector was using a stever bar (pry bar) to leverage motor base.
The employee was in the process of installing a roof bolt. As employee raised the head of the roof bolter up to meet the roof bolt that the employee was holding in place, employee accidentally engaged the rotation on the drill pot causing the bolt to spin and fracture employee's right thumb.
Removing high tension spring, pulled hook through finger; required a couple stitches. Back to work same day.
The employee was descending a step ladder in 10 bay of the pit shop. When they reached the second step and stepped down, they missed the first step and fell backwards onto their outstretched arm resulting in a sprained wrist. The ladder also fell to the ground.
EE was in high reach when the lift strap slipped and they fell into side of man basket bruising thorax.
The employee used a piece of scrap floor grating in an attempt to dislodge material blocking the discharge chute of the dryer bag house. The metal twisted in the employee's hand, causing a laceration of the right index finger.
Employee was lifting a rigging cable onto the storage hooks when EE felt pain in stomach and a small bulge. Employee was seen on by the Dr. on 8-29 and released without restrictions. During the followup appt on 9-18 employee released to modified duty.
Injured strained back after throwing cable over underground. Was taken for medical attention and was placed on restrictions.
DURING PRE-START UP INSPECTION OF SECO BOTTOM SHAKER SCREEN, EMPLOYEE WAS ON DECK CONDUCTING INSPECTION. THE EMPLOYEE LOST FOOTING DUE TO DAMPNESS OF SCREENS, RESULTING IN A FALL, WHICH INJURED THE EMPLOYEES RIGHT KNEE.
A miner suffered a sprain to EE's lower back while shoveling. The miner states EE was doing clean up and when EE lifted the shovel EE felt pain in EE's back. EE reported the incident however did not seek medical treatment until the following day.
EE was operating a scoop in the No.8 heading on the No.3 section. EE hit a hole bouncing the employee around in the deck jarring their back.
Employees were unscrewing a bowl from the crusher mainframe. Employees were using bars to unscrew the bowl. The injured employee's bar slipped, causing the employee's right hand to come in contact with a hand rail. Pinching the employee's finger between the bar and hand rail.
Miner was moving 6 inch discharge hose. PC650 trac hoe operator was helping to move hose. The operator lowered the bucket of the trac hoe on the miners left foot.
Employee was moving a 4 inch x 4 inch wooden block and received a splinter in finger. Employee reported injury on 8.30.2019. Employee refused medical attention. Employee returned to work on the next regular work day on 9.3.2019 and stated that they had visited a personal physician and was prescribed antibiotics. Finger became infected.
EE was shoveling material spilled at the plant next to an I-beam for the screen deck. As the EE shoveled, they struck the I-beam with the edge of the shovel causing their right wrist to bend backwards, causing a muscle strain.
EE was changing a transfer hose when EE's left hand was pinched against the adjacent hose causing a hair line fracture to the left index finger
Operator was trying to align a chute and sliding collar in place, pushed too forcefully, and felt a pain in right arm/elbow. Iced and cared for. Did not fully improve over the course of a month. Results now show partially torn biceps muscle.
The employee was setting the grease on the jaw. Employee left the working surface and slipped and fell 2-4 feet. which results in the injury to their foot.
Employee was cutting a splice on shuttle car when box knife broke. Momentum caused knife to cut left pinky finger. Employee returned to work with restrictions.
Employee claims injury happened sometime during the shift. Employee does not know what task they were performing or what time the injury occurred. Employee did not initially report the injury, it was reported on 08/26/2019 at approximately 11:40 AM.
Employee was using a 6lb hammer to knock mud off of a haul truck and afterwards felt pain in EE's right elbow.
Haul truck operator was about to dump load at the dump point. EE reached down for the dump lever and felt a pop in the shoulder and pain thereafter.
The accident was a trip accident. There was a routinely used item of lift equipment. The employee moved between the equipment and the wall, forgot about the lift equipment and tripped on the base of the lift equipment when EE turned 180 deg. away from the wall. The employee will be more aware of EE's surroundings. The employee suffered a sprained little finger and a few scrapes.
Employee was sitting in haul truck and another employee backed into the truck.
Crawling under truck working on transmission for an extended period of time. Resulted in swelling of right knee.
Party injured left shoulder while trying to lift the lid on one of the sand-trucks. Lid was bound close and the 3ft' pry bar-like tool was used. Employee stated EE may have jerked back EE's arm to hard and felt EE's shoulder pop. Later that night it started causing EE pain.
Miner was in the process of scaling down the loose ground on the rib, as EE moved to avoid falling materials EE twisted left knee.
Employee conducting maintenance on broken screw. Applying tension w/pipe wrench & come-a-long, during prep for welding - the wrench slipped causing employee to fall backwards, hitting elbow on guardrail. Injury non severe so employee continued to work. Employee realized the next morning that the pain had not gotten any better and pursued further evaluation.
EE felt pain in abdomen when lifting a 16' drill steel. EE placed on restricted duty on 8/22/2019 while testing was performed. Testing was negative for hernia and EE was released back to full duty.
A pry bar was used to loosen material on feeder and sides in a chiseling motion and employee's hand slipped being cut by the handle of the pry bar.
Employee stated EE pinched finger between the boom and the basket when the stop block broke off and allowed the boom to lower more than typical.
Department Helper cleaning south basement with bobcat, while exiting the equipment stepping down felt a sharp pain on left foot left side of heel.
Employee was getting out of vehicle when EE's hand slipped from the shuttle van door's window pane. EE's finger caught on the door's handle when it slipped
Employee was tightening a hose connection on a hydraulic hose when employee felt a sharp pain in neck.
While performing repairs to a conveyor drive coupling, the injured employee stepped onto a motor base, approximately 8-10 inches in height. Foot slipped off the ledge of the base, causing right ankle to twist under body weight, resulting in a fracture to the right fibula.
Employee was carrying a pump through mud and slime, and EE slipped. EE had no initial pain. About 3 hours later, EE felt pain in the left knee, couldn't walk on it. EE saw a doctor the next day. EE was put on crutches and was sent for x-rays. EE was referred to an orthopedic surgeon for further evaluation.
EE was operating a haul truck when it started to smoke, and the employee was exposed to some of the smoke. EE was seen at the ER and diagnosed with bronchospasm. EE was released to restricted duty for two days.
While repositioning a strobe light on a light vehicle, employee missed step on the running board and awkwardly put weight on left knee feeling a pop and pain. Employee reported the incident to supervisor the following shift. Employee was seen by the safety group and referred to the clinic for further evaluation. Was placed in a knee brace.
Rubber Tire Dozer 3 was cleaning up shovel 1 when EE hit a bump in the pit which resulted in the operator getting jolted around in the cab causing injury to EE's lower back.
Employee was cleaning the preheater tower using the water blaster. During this cleaning a large chunk hit water blaster lance causing it to twist while employee was engaged with gun trigger. The trigger part of the gun also twisted along with employee's left hand ring finger, causing a fracture of tip of left ring finger.
EE sprains knee when EE felt knee pop while dismounting water truck. EE transferred weight from one foot to the other on factory truck steps and lost balance, falling to the ground.
Lifting a piece of Curb with the overhead crane form the conveyor the East button was pushed instead of the up button and the piece slid out and struck the operator's right hand
Employee stated that as they were loading a blast hole, a small rock fell from the hole being loaded above and struck them on the neck and shoulder.
Employee climbed a fixed ladder to access the top of FM2 DC when they felt a pull in right shoulder. The incident happened around 11am. The initial injury was ruled First Aid for muscle strain (non-MSHA reportable.) EE was placed on light duty to rest. Further evaluation over the past 2 months, lead to physical therapy making it a medical.
EE attempted to stop a pull behind trailer from rolling and strained their back area.
The employee was underground and preparing to load used conveyor belt structure in a scoop bucket. The employee pulled the scoop up next to the conveyor structure and exited the scoop. At this point they felt pain in the middle and lower parts of their back. Medical examinations reveal extensive arthritis in the employee's back.
EE had clocked out at the end of shift and was exiting the building via stairs. It had recently rained and the EE slipped and fell. Bruising the left triceps and left mid-back.
Miner exited pick-up truck and while closing door pinched right index finger breaking the tip of EE's index finger.
Employee was assisting moving a lifter toward the center of the mill to fit in the required location. Another employee was hitting the lifter with a sledgehammer to push a bolt out of the bolt holes and the first employee's thumb got in the way causing it to be hit with the sledgehammer.

EE was checking AC on Grader when EE reached to grab handrail and missed and fell off machine landing on left side injuring left elbow. EE continued to work until 08/25/19 where EE visited ER and was dx. with fx. of left elbow and referred to specialist on 08/30/19 where a full arm cast was applied to left arm.
Employee was operating a gopher drill on the 5A Section belt when a piece of rock came from the mine roof and struck employee's right hand causing a laceration to the right middle finger. Employee received 13 stitches.
Employee was climbing down the ladder of a 980K loader when the bottom rubber step gave way while performing routine maintenance. Employee fell approximately two feet to the ground striking back and left wrist.
Employee stepped off of bolter to ground below rolling ankle on a piece of coal.
While hosing off excess material from a portable crusher a piece of foreign matter entered employee's eye.
Employee was riding on a rubber tired mantrip when a piece of belt struck the rescuer box, dislodging it and pinning employee's right foot and ankle. Employee later went to Dr.'s office with swelling and was diagnosed with a contusion and restricted employee's activities.
A Miner suffered a strain to EE's right knee while traveling by foot. The miner states EE slipped but did not fall while carrying a 5 gallon pale of oil to the E5 gearbox. EE has been placed on work restricts after a follow up visit to the clinic.
Employee was working in the bagging/packaging area. The table for bags dropped and left a 100 pound bag of material suspended. Employee opened the door to catch the 100 pound bag before it fell on the floor. As employee attempted to grab the bag, the 100 pound bag pulled EE's left arm backwards. Employee started physical therapy on 10/15/2019.
Employee climbed steps to the 1/2 and 3/8 belt platform to get a rock sample. When EE stepped onto the platform, EE rolled EE's right ankle and had to catch self.
EE was putting a piece of scrap metal from the crusher (45-037) into the steel scrap hopper. While throwing it into the hopper EE felt something in the left elbow area give and a burning sensation started on the inside of EE's arm.
Employee was loading a piece top structure into a scoop bucket and strained their back.
While exiting the water truck, EE felt a 'pop' in EE's right leg and pain in EE's ankle. EE fell to the ground between water truck and maintenance truck. EE got up and leaned against water truck but then collapsed to the ground, hitting EE's head on the passenger side running board of the maintenance truck. EE was taken to the hospital and found to have a sprained ankle.
Driving water truck daily. Employee developed back pain over the course of several days and reported it on 8/12/19. Back strain.
Employee was attempting to load feeder, reached to top of feeder and felt a strain in right shoulder.
EE was dismounting the D11T cat dozer when EE lost footing and step to the ground twisting the left knee.
An employee parked a truck underneath the muck chute to load kiln brick. The steps of the truck were even with the edge of a concrete pad with a two inch difference to the ground. Employee stepped out of the truck, foot was not squarely on the even surface. As they put full weight on that foot, it rolled to the lower surface causing a sprain to the ankle.
At approximately 6:45 am EE was getting into 503 man van. While getting in, EE hit EE's left knee on a seat spring that is sticking out of the seat cover.
The water truck operator was not able to clear a truck fall protection rack that was left down by an outside truck driver. When trying to raise the metal rack, their hand was pinched between the rack and water truck mirror.
The 1st tooth on the loader bucket made contact with the rear side of the canopy of the haul truck. The truck operator felt a jolt, and experienced lower back pain. The employee received treatment and was cleared to return to work by occupational medicine. On the employee's first day back at work, employee experienced additional pain and was given restrictions.
The employee added oil a machine and then climbed into the cab to turn it off. After turning off the machine EE proceeded down the ladder of the machine slipping on the second rung falling approximately 4.5 feet to the ground landing on EE's buttocks. When EE got up EE felt a sharp pain in EE's pelvis and lower back causing EE to call for help as EE was alone.
Haul truck trainee and mentor were being loaded at the 40 shovel when on the second dipper, a rock rolled and made contact with the side of the bed. The operator stated they felt back pain after they left the shovel.
At approximately 17:15 EE and co-worker were performing a bit change on the 587 drill. EE was in a bent over position while trying to pull the bit basket straight out of the deck. After several attempts EE felt back and neck tense up and start to get sore
EMPLOYEE STEPPED ON A LIFTER INSIDE OF THE SUPER SCRUBBER AND EE'S RIGHT FOOT SLIPPED OFF THE SIDE OF THE LIFTER. EE HEARD SOMETHING SNAP IN EE'S RIGHT ANKLE.
Employees were installing a wash water line on the opposite side of existing wash water line. The job was finished and they started to charge the water line. During that time the line swung upwards and pinned the finger of one of the employees between the tunnel roof and vent pipe. EE was taken to clinic and received stitches.
EE was using a 4 foot steel drift to drive out the anchor pin on TD524 l/h ripper tilt cylinder. IE was stabilizing the end closest to the bore with left arm. When the drift came out of the bore as EE was driving it IE was not ready for it and the weight of the drift pushing down on IE's arm caused pain in the left shoulder. Left Shoulder Strain.
Mixer operator irritated lower back while lifting liquid pigment into mixing pot.
EE was removing the screw cover for the bag house screw. After removing the bolts the EE discovered there was a tack tac weld. The EE took a 6"" angle grinder to remove the weld as the wheel contacted the weld it kicked back striking the EE in the Right thigh causing a laceration.
EE struck elbow on a forklift that was parked behind EE when EE lost grip opening the discharge spout on a super sack while filling a machine.
EE claims injury occurred on 8/5/19. EE reported to Supervisor until 8/25/19. EE was taken for medical eval on 8/26/19, released to full duty. On 9/10/19 EE complained pain returning and was transported for second medical eval and was released to full duty. MRI was scheduled & was obtained on 10/10/19. 11/5/19 EE returned for further medical evaluation, assigned restrictions.
Removing Bell Housing bolts to engine. Crane Sheave Block sprung back and struck left hand causing laceration.
Miner was exiting the excavator # 2775 walking southwest towards the haul truck #1517, when miner stepped on a mid-size rock. The rock shifted causing the miner to fall backwards to the ground.
Employee was moving a water hose for a C/M from mud, when the employee felt pain in lower back. The injury occurred in wet & muddy conditions. X-Rays were negative and employee was diagnosed with a strain.
While changing a belt on a conveyor, the employee lost control of the guard on the head pulley. The head pulley then struck the employee in the right arm causing a laceration.
Wheel came off truck causing truck to hit rib. EE placed on restricted duty after medical treatment on 8/3/2019
Employee got finger pinched between anvil plate and housing a pile driver. Was helping crane operator get the pile driver position on concave for removal from 30"" crusher.
Employee was walking down a hill and turned abruptly to talk with someone behind them and states that the pivoting motion caused a ""pop"" in their left knee. Employee describes lost range of motion but no pain. Employee had the knee wrapped and went on light duty for the rest of the day, at which time they would go to a local clinic.
An employee's finger was caught between a machine guard panel and a belt's framework above when another employee attempted to lift another panel into position with a skid steer loader, resulting in a laceration requiring stitches and light duty.
Maintenance employee's leg and foot caught under personal vehicle and HDPE pipe, that Maintenance crew were dragging.
Employee was cleaning slitter box when the door came down pinching a pinky finger on the left hand. It required a number of stitches to treat the injury.
Employee was walking down the stairs. When EE stepped to ground level EE twisted EE's left ankle.
Haul truck was backing truck up to the berm. While the truck was lowering the bed, driver noticed the outside of cab was engulfed with flames coming from the engine area
WHILE PINNING AND PLATING THE BAUXITE WEIGH FEEDER, WAS SWINGING SLEDGE HAMMER AND IT GLANCED OFF AND DOWN SMASHING EMPLOYEE'S FINGER INTO THE LIP OF THE BAUXITE DOOR
Employee was pulling drip line when UTV pulled up and turned off. Employee grabbed lines and cut them, causing them to snap back while employee was still holding them. Employee then felt pain in lower left arm
EE climbing onto machine, EE slipped and fell onto right side. EE placed on restricted duty on 8.1.19.
EE felt pain in arm and shoulder after performing 28 sieve sizings in a work shift. Medical treatment was sought where EE was given an anti-inflammatory shot as treatment.
While stepping out of a pickup truck, the employee stepped on a rock and felt a sharp pain in the middle sole and heel of the right foot.
Employee dropped part on EE's hand causing laceration.
Injured was roof bolting on the M4 travelway and while installing a roof bolt pinched EE's thumb between the wrench and the roof plate. Injured received a laceration from EE's injuries which required stitches.
EE was trying to fix a material leak that was dusting out of a hose. The leak changed direction and blew cement in EE's face.
EE was changing belts on dredge when the wrench slipped and EE cut EE's left arm on hose clamp
Employee was loading supplies on the roof bolter when a strap slipped cutting thru EE's glove and causing a laceration on the thumb.
Employee removed cover from dredge pump. Placed cover on tube and it fell off onto employees right foot.
At approximately 4:30 pm August 30th, 2019; A piece of steel was being cut out of the chute with a sheave rope attached to control the downward motion when the piece was cut free. When the piece came free the rope tensioned and kicked toward an employee who was standing too close to the ""v"" of the rope striking EE's arm and face.
EE was driving in a wooden wedge, the wedge was at an awkward angle. When hammering with the right hand, EE missed and hit pinky finger on left hand. EE got 10 stitches on pinky finger.
EE was stepping back to grab a radio and tripped self. EE fell back caught self with right hand. EE has been issued a splint from the DR. for a sprained wrist.
Employee was attempting to start a thermal fluid heater. After multiple attempts the thermal fluid heater fired causing the control panel to fly open hitting the operator in the face causing a hairline fracture to the nose and five stitches.
Employee was welding. A piece of metal fell onto a can of spray paint. It punctured and caused a flame. The flame burned the leg of the employee.
Ee was unplugging a sump pump in the lime slaker area. Air broke the plug free and lime dust and debris was blown under the employees safety glasses into eyes. Seen at the ER and diagnosed with chemical conjunctivitis. Ee prescribed medication for the eyes. Released to full duty.
While driving a top of stock pile hit a hole with rear tandems. When hit hole, seat bottomed out and started having pain. Drove to a road market and called truck boss.
the employee was behind the bolter reaching for a piece of tie wire with right hand, had left hand on top of the operators deck canopy the bolter slightly moved, it hit a high spot on the bottom, causing it to raise up mashing left index finger between the canopy and the top causing a fracture.
Employee was tightening lug nuts on roof bolter and ratchet slipped off and EE fell into roof bolter, busting EE's lip and cut EE's thumb.
Employee was driving a haul truck. Employee hit a bump and was jarred in the seat. Employee reported feeling lower back pain and went to the dispensary for evaluation.
EE was opening a harness wire box with a knife when the knife slipped and EE cut middle finger on left hand. EE received butterfly stitches in the finger.
Employee was working on a locked out fan. The employee wedged a wrench in the fan blades to keep them from moving while EE worked. The fan blades are being pushed by high air pressure in the system. Wrench slipped causing employees hand to be pinned between the wrench and the base of the fan.
The employee was reaching to turn off the water hose that the employee had been using to wash out around a conveyor belt. Employee lost balance when employee reached to turn off the hose. Employee fell over against a newly poured concrete wall. The concrete form wires had not been properly removed and there was an exposed wire left sticking out that cut employee's arm.
Employee was walking across the section and raised EE's head up slightly hitting the mine roof with EE's hard hat and EE's teeth struck together resulting in a possible cracked tooth. The ER doctor released EE after EE's exam, and EE followed up with a dentist and the dentist determined that the best preventative action would be to remove the tooth.
EE was bolting top and turned to look under a brow. Something came down and hit EE in eye. Object was able to get behind glasses because of the way the EE had their head turned. This is being reported due to EE being prescribed antibiotic Eye Drops.
Employee was reaching across the table and a rock fell onto the back of EE's left lower leg resulting in a contusion. EE was taken to Occupational Medicine for treatment, advised to elevate the leg for the remainder of the day and return to full duty the following day.
A piece of torch slag fell through a hole in pants into a slip on boot. It burned a spot below right ankle. EE has worked every day with no restrictions. EE went to the doctor on 8/29/19, given a Tetanus shot and oral antibiotics as a precaution, after the wound was cleaned, dressed and inspected by the doctor. Additional visits if needed.
Employee was using a slate bar to remove a broken roller. The bar slipped as EE was prying the roller out and pinched EE's right ring finger between the bar and the belt structure. Employee was treated with 4 stitches.
The employee was using a vacuum truck performing normal cleanup duties. The employee felt EE had strained EE's back which EE said has occurred several times before. A third party injury care administrator prescribed self care of ice, heat, and ibuprofen. The employee was doing better but on 9/19/2019 the employee elected to go to a chiropractor making it recordable.
EE was working in a loader and was stung by a bee, which EE is allergic to.
Employee received chemical burn on chest while changing screen cloth.
Employee was holding a piece of sheet metal while another employee was drilling a hole. The sheet metal slipped and cut the right thumb.
EE was placing a spill board on to the side of the unit tailpiece. While doing so, EE caught left thumb in between the spill board and the unit feeder. EE continued to work and turned an accident report in 9/9/2019. Then on 10/7/2019, EE went to see an Orthopedic Surgeon who informed EE that the thumb was broken and to start wearing a brace on the thumb.
While waiting for scoop to stop in order to unhook pull rope from bucket, the employee slipped off a ledge and was caught between the ledge and scoop bucket.
The employee cut EE's finger in two places while removing spud pins from the starboard spud. The cuts required 13 stitches, total. Gloves were being worn at the time. The stitches were done by MedExpress in NLR.
Pushing a small rock while bolting and bend back on left thumb. X-rays were negative and EE came back next shift. They gave EE a splint to wear on thumb.
Ie was bent down moving the water hose to the continuous miner when a rock came out from behind the mesh hitting the employee in the head slamming the employee's face into the shuttle car and fracturing a tooth.
While brush cutting with tractor and 15 foot brush cutter operator hit an unseen rock which was propelled from the cutter through the back glass of tractor striking the operator on the right side of head causing a laceration. Operator was transported to clinic where laceration was stapled shut.
Employee was handling belt clamp when one side dropped causing EE to catch EE's finger. Cause of this incident was lack of employee concentration. PPE was not a factor, mining equipment/mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency may have been a factor.
Employee was checking coolant in a vehicle. The coolant sprayed when EE opened the radiator causing EE to fall and hit EE's shoulder.
Welder was welding on 6040 Bucket in the shop all day with no complaints of eye irritation. Welder finished shift, went home and woke up from sleeping at 10:30 pm complaining of eyes irritating. Took employee to doctor where employee was diagnosed with arc flash burn.
Employee was roof bolting in the 1st North Rock Slope, employee was putting a 5 foot pusher steel into the 3 foot extension steel and as employee was holding the chuck wrench while pushing the steels into the hole, employee smashed right little finger between the chuck wrench and the clamps on the drill head of the roof bolter.
Employee felt something in eye when EE and others were in the process of installing a vibrator on a sand bin. A foreign object was removed from EE's eye at the emergency room and EE was returned to full duty.
The drivers were told by the site foreman to dump anywhere deemed safe. When the driver went for the first dump, EE backed against berm and attempted to dump. It was slightly uneven ground and the dump gate locked so they tipped over. Had to be helped through windshield. Dump truck had to be towed away. Ambulance was called. EE sustained cut on head and bruised arm.
Employee was working in feeder area. Employee lost footing, fell forward, and struck forearm, causing a fracture.
Employee stated that EE pinched finger between two pieces of metal resulting in a laceration.
The employee was helping to change out a belt on the overhead conveyor. As EE was holding the shaft, it slipped out of EE's hand pinching the right hand between the frame and the shaft. The 4th finger on EE's right hand was smashed and had a laceration from the impact.
IE was helping install and stand up steel sets. As they were raising the steel set, it rubbed the rib causing a rock to fall. This resulted in a laceration to the left ear requiring stitches.
Employee was repairing a loadout spout. While moving the spout the employee's finger got pinched between work platform and the bottom of the spout causing a cut on left hand ring finger that required 3 stitches.
Wasp sting left forearm.
The EE was cutting a notch into a section of belt so that a splice could be identified that needed replaced on a idle shift. The EE stated that as they were cutting the notch, the knife slipped and cut their left index finger.
Team member stepped into a divot causing the team member's knee to jam. Initially this injury was considered first aid. Physical therapy was requested on 9/19/19 making this reportable.
Employee was roof bolting, when employee lowered the pinner boom it hit a pinner steel that had fallen out of the pin tray causing it to flip up and hit employee in the mouth and nose resulting in a fracture to nose.
Changing cutting edge on loader when bolt head spun and cut operator's left hand.
Employee was handling rail ties with a pry bar. The bar slipped and caught employee's finger between rail and bar causing injury.
Employee was welding underneath truck with welding helmet and safety glasses on when buildup dirt fell from the bottom of the frame and dropped down onto the shield and scratched their left cornea.
On Wednesday 08/21/2019 this employee was working at crusher site picking rebar from conveyor. On Thursday 08/22/2019 employee told supervisor that it was painful to breathe/sore throat/stuffy nose. Employee said it was the dust from the crusher and wanted to go to the doctor.
Employee was sitting at desk and felt a sting on hand, looked down and saw a spider on the hand. Employee went to doctor the next day and was treated for an infection.
Employee was changing out a bottom roller on conveyor, when the roller fell on the opposite side of where employee was holding roller, causing the end which employee was holding to go up and pinching employee's left thumb between the roller and the conveyor frame.
While employee was tossing an oil bucket, EE's finger was cut when a jagged edge contacted EE's finger causing a small laceration that required two stitches.
EMPLOYEE WAS TIGHTENING A HOSE CLAMP WHEN IT SLIPPED AND CUT LEFT HAND REQUIRING STITCHES.
Employee was shoveling the 2nd 54 north belt line when left hand slipped off of the shovel causing EE's thumb to come in contact with the bottom roller frame resulting in a fracture of the left thumb.
Employee received a laceration to the left ring finger while removing a bolt from the end of a hydraulic take-up jack. Once the bolt was removed, the jack fell and caught EE's finger between the jack and the storage unit framework.
Employee caught knee between tire and bumper while putting on alternator on Truck. Put pressure on knee and or twisted knee cap.
As employee proceeded to check a feeder EE stepped on uneven ground and stepped wrong and EE's knee popped.
While in the #4 entry the employee dropped the drill pot down on the roof bolter when a piece of drill steel fell out of the hole hitting the ground. The employee bent over to pick up the piece of drill steel when the other piece fell out striking the employee on the left pinky finger. The employee then went to the hospital where it was determined that the finger was fractured.
Employee was moving a 12' jackhammer bar during a downtime. As the employee was moving the bar, the tail end of the bar struck dryer crusher, which caused the employee's left wrist to be bent sideways sharply. Treated with OTC meds until not getting better. Brace and prescription meds given 8/27/2019.
EE was tacking a liner on a loader bucket when a piece of weld broke off and struck EE on the right side of the forehead. Seen at clinic and stitches were required to close the laceration. Released to full duty.
Employee pinched right index finger between the roof bolter chuck and the mine roof. This resulted in a laceration and required sutures.
Employee was shoveling on belt when a piece of foreign debris got into EE's left eye resulting in medicated eye drops. Cause of this incident was employee concentration. PPE was not a factor, Mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency may have been a factor.
Employee was using a razor blade to cut out a belt clip. Employee was using left arm to hold the belt while cutting with right hand. Employee slipped, and the blade cut left forearm. Employee received six stitches, and was released back to work.
Employee pinched EE's right index finger between a hammer and a drift causing a laceration.
Employee not following procedure climbed 6' ladder that was not level, employee was not wearing PPE gloves, the ladder tipped and employee cut finger.
The employee was crossing a stationary flat car when the employee slipped. As employee slipped, employee put a hand out to catch self and cut a finger on a steel band holding belt structure. It did not include any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
EE was pulling a curtain off of the mesh when a nail holding the curtain to the mesh came out striking EE in the mouth breaking the tooth.
Employee was riding in a CAT 745 haul truck while training. While going through a rough area the employee reached forward to grab the hand hold. EE's hand contacted the hand hold with some force and caused discomfort and swelling in EE's left wrist. Employee was taken to the Work Med Clinic and was diagnosed with a non displaced mid scaphoid fracture in the left wrist
Employee was trying to secure 3"" female camlock fitting to 3"" male fitting. While holding the hose in left hand, employee opened the valve. Unexpected pressure on the hose caused the fittings to come apart and cut employee on the right forearm
Mechanic was grinding off a stripped bolt when grinder jumped and struck left wrist causing a 1 - 1 1/2 inch cut which required sutures to close.
Employee was laying down a water pump that weighed 86 lbs. As they were laying the pump down their right hand ring finger was smashed fracturing a bone in the finger.
EE attempted to mount the water truck & failed to obtain 3 points of contact. EE placed right hand on the handrail, then attempted to place right foot onto the lowest rung of the ladder. EE's foot missed the ladder rung causing EE's body to fall forward swinging to the left. EE struck left ear causing a laceration resulting in 25 stitches.
During cleanup after a lime kiln de-slagging job an employee got lime kiln dust in the right eye from debris that fell from above EE.
Employee was operating the loader in the east quarry, cleaning up the remaining rock on the quarry floor when they ran over a 2 ft. diameter rock that fell out of the loader bucket. The loader front tire ran over the rock causing the loader to rock to the left, pushing the employee into the ROPS inside the cab, hitting their head, causing a cut to the head requiring stitches.
Tech was replacing a bearing race on conveyor tail shaft. Tech used a die grinder to cut the race and then used a hammer and chisel to wedge apart the race to drive off shaft. A metal piece of the bearing race or chisel chipped off and embedded in the side of the hand. The small (approx. 3cm) metal piece had to be surgically removed at the doctor.
While removing starter on drill, EE dropped socket down to belly pan of drill. EE crawled under drill to loosen belly pan to retrieve socket. EE loosened bolt too much, therefore the belly pan dropped down on one end striking employee on the head. EE had temporary swelling, dizziness and headache. EE was examined at hospital and released back to work 8/19/2019.
Employee parked water truck and was descending the stairs when they missed the last step of the ladder and fell backwards onto the ground hitting their head. The employee received 3 staples to the head.
Moving the wash down hose across the top of the monorail. The wash down hose got stuck, when EE tugged on the hose it came on and whipped, striking EE in the mouth causing a broken tooth and stitches.
While changing the battery on their ram car the employee dropped the battery lid on their finger.
EMPLOYEE WAS INSTALLING A TRAM MOTOR IN A SCOOP. COMEALONGS WERE BEING USED TO LIFT AND POSITION THE MOTOR. THE MOTOR SLIPPED OFF OF THE FRAME RAIL ON THE SCOOP AND FELL INTO THE CRADLE. THE EMPLOYEE'S WRIST WAS POSITIONED IN AN AREA WHERE THE MOTOR STRUCK IT CAUSING IT TO BE PINCHED BETWEEN THE FRAME AND MOTOR COUPLER. EMPLOYEE WAS TAKEN TO HOSPITAL AND RECIEVED 6 SUTURES .
4900-334 bolting back - rock came out of back hitting miner on upper lip
An employee was grinding a weld on the Rev Deck of the 16 Shovel when debris flew under their hood and safety glasses. The employee had metal embedded in the cornea of their eye.
Adjusting forks on a forklift and one of the forks slipped and lacerated the injured worker's right thumb
Employee using a pry bar to move a door on the tertiary damper out from its frame so they could inspect the tamper. It was almost out when the employee gave a final burst of force to free it, the bar slipped and the employee smashed EE's middle finger. 9 stitches in upper finger.
*****did not go for treatment until - 9/24/2019*** Riding in the passenger seat of a bulk truck, truck started jumping attempting to get up a short ramp in the sand, thrown up twice in the seat, legs became sore.
IW leaned 36"" pipe wrench against a pipe. IW placed left hand on the floor to support themselves as they repositioned a brace. Pipe wrench fell over striking employee's hand. IW sustained fractures to the left middle and ring fingers. IW finished their shift and received no lost time.
EE was working to change skirting material from a skirt board, when the utility knife EE was using slipped off of the skirting material striking the EE on the right forearm causing a laceration that took 8 sutures to close.
The employee suffered a deep laceration on the left side of the top of EE's head resulting in 7 stitches.
Assisting mechanic repairing haul truck. Loosening a bolt, slipped, and cut right index finger. Three stitches required.
