HOIST OPERATOR COMPLETED TESTS OF ALL HOIST SAFETY FUNCTIONS. WHILE TESTING THE EMERGENCY MODE OF OPERATION, THE BRAKING SYSTEM FAILED TO RE- SPOND AS DESIGNED. DAMAGE TO MANCAGE, HOIST, AND SHAFT, TO BE DETERMINED.

A ROOF FALL WAS DISCOVERED IN THE E-3, 2ND NW SUBMAIN 7 + 50 MEASURING 5O'L, 20'WIDE, 6 1/2'HIGH. THE FALL WILL NOT BE CLEANED UP, BUT TIMBERED AROUND. THE FALL WAS INVESTIGATED BY MSHA.

EMPLOYEE OPERATING LOADER WAS MOVING STOCKPILED MATERIAL FROM ONE AREA AND THEN DUMPING IT OVER THE EDGE ON AN ESTABLISHED STOCKPILE. BEFORE BE-GINNING TO DUMP, THE OPERATOR PUSHED OFF THE EDGE AND THEN BEGAN DUMPING MATERIAL OVER THE EDGE(PART ON TOP AND THE REST GOING OVER THE SIDE). THE BANK UNDER THE RIGHT TIRE GAVE WAY. THE LOADER SANK AND THEN ROLLED OVER. NO INJURIES.

DURING MSHA'S INSPECTION OF THE SHAFT, THE HOIST WAS BEING RUN IN THE MANUAL MODE. AT THE SHAFT LANDING, THE HOIST WAS RETURNED TO THE AUTOMATIC MODE WHEN THE COOLING FAN FAILED TO START. THE MINE WAS EVACUATED AND A CABLE WAS REPAIRED. THE HOIST WAS TESTED AND RESTORED TO SERVICE AT 5:20 PM.

HOISTMAN CAME TO THE MINE AT 2100 TO DO PRESHIFT CHECKS FOR GRAVEYARD SHIFT WHICH STARTED AT 2300. HE STARTED TO DO HIS HOIST CHECKS WHEN #2 HOIST KICKED OUT. IT WAS RESET AT 2210 AFTER CLEANI NG CONTACTS & PERFORMING HOIST CHECKS. THE HOIST KICKED OUT AGAIN & WAS RESET. ELECTRICIAN & HOISTMAN WERE IN HOIST GOING UNDERGROUND WHEN IT KICKED OUT AGAIN. THEY WERE TRAPPED FOR ABOUT 45
THIS MINE WAS FLOODED DUE TO 5 INCHES OF RAINFALL IN A VERY SHOR PERIOD OF TIME.
A ROOF FALL HAD OCCURRED IN E-4 (19+85) IN THE 1ST NW SUBMAIN MEASURING 30'L, 20'W, 6 1/2'H. THE FALL WILL NOT BE CLEANED UP AND WAS TIMBERED OFF.
A ROOF FALL OCCURRED ON THE 3 WEST BELT AT #7 CROSS CUT. THE FALL WAS APPROX 4' HIGH BY 20' BY 40' LONG. THIS AREA HAD BEEN BOLTED WITH 4' RESIN BOLTS.

EE OPERATING THE CONTINUOUS MINER IN THE #5 ENTRY WHEN A PIECE OF ROCK BROW FELL STRIKING HIM FROM BEHIND RESULTING IN FATAL INJURIES.
A FATALITY OCCURRED AT THE WINDFIELD SOUTH MINE ON AUGUST 21, 2001 AT APPROX. 11:30 A.M. MECHANICS HAD BEEN ASSIGNED TO TURN THE DRAG ROPES AND PUT ON ONE DUMP ROPE ON THE D-22 DRAGLINE. THE D RAGLINE BUCKET WAS CONFIGURED SUCH THAT THE ARCH TO EAT. A CLOD OF DIRT FELL FROM THE BUCKET. APPARENTLY STRIKING THE VICTIM ON THE BASE OF THE NECK. EE ON THE SCENE IMMEDIATELY ATTEMPTED TO
EE WAS ASSISTING IN MOVING 5 CONNECTED RAILCARS TO AN ADJACENT TRACK BY MEANS OF A WHEELL TRACTOR. THE REAR RAILCAR BECAME UNCOUPLED FROM THE TRACTOR. THE EE SIGNALED THE OPERATOR OF TRACTOR T O BACK AWAY FROM THE REAR RAILCAR TO ALLOW RECOUPLING. AFTER THE WHEEL TRACTOR BACKED AWAY, THE EE POSITIONED HIMSELF BETWEEN THE TRACTOR & RAILCAR. THE SLACK RELEASED FATALLY INJURING EE.
EMPLOYEE WAS STANDING BESIDE A PIECE OF WIRE ROPE LYING ON THE GROUND WHICH WAS ATTACHED TO A BELT COUNTER WEIGHT WHICH HAD BEEN RAISED TO AN ELEVATED POSITION AND SECURED. AN0THER CABLE WHICH WAS BEING USED TO PULL THE SLOPE BROKE, AND SUBSEQUENT MOVEMENT CAUSED THE COUNTER WEIGHT TO FALL, PULLING IN THE ATTACHED CABLE WHICH AMPUTATED THE LOWER RIGHT LEG OF VICTIM.
EE WAS SIMPLY BENT OVER/WALKING UNDER A LONGWALLSHIELD. WHEN HE FELT PAIN IN HIS LOWER BACK
EE WAS LIFTING 3 OXYGEN & 3 ACETYLENE BOTTLES ONTO THE BACK OF A PICKUP TRUCK WHEN HE FELT STRAIN IN GROIN.
CHANGING PULLEY BELT FINGER ON PULLEY SLIPPED & FELL.
EE WAS REPAIRING A HOLE IN A 10 INCH DIA RUBBER WATERLINE. EE BENT OVER TO WORK ON RUBBERLINE. HE PICKED UP A SECTION OF THE HOSE AND FELT A PAIN IN HIS GROIN AREA.
WHILE CHANGING CUTTING EDGES ON A 988F LOADER EMPLOYEES WERE USING A ROUGH TERRAIN FORKLIFT TO MOVE THE EDGES. INJURED EMPLOYEE WAS ADJUSTING THE CUTTING EDGE ON TO THE FORKS AND PLACED HIS LE FT HAND ON THE TOP OF THE FORKLIFT BACK REST AS THE FFORKLIFT OPERATOR TILTED THE FORKS BACK, PINCHING THE EMPLOYEES FINGER BETWEEN THE BACK REST AND THE BOOM HEAD. FAILURE TO COMMUNICATE BET
USING A RAM (500 LBS.) THAT WAS SUSPENDED FROM A CRANE TO LOOSEN THE HEAD NUT ON A SHORTHEAD CRUSHER. WHEN THE RAM CONTACTED THE WRENCH THAT WAS WELDED ON TO TO NUT, THEWRENCH CAME UNCOUPLED A LLOWING THE RAM TO FALL TO THE GROUND AND CONTACTING THE SAFETY SHOE OF THE INJURED EE CAUSING PARTIAL AMPUTATION OF THE LEFT BIG TOE.
EE WAS TYING A PORTABLE FAN AT THE SECOND STAGE PREHEAT VESSEL. THE ROPE GOT ENTANGLED IN THE FAN AND WRAPPED AROUND THE END OF HIS LEFT RING FINGER. THE LITTLE FINGER ON HIS LEFT HAND WAS ALS O INJURED. THE RESULT WAS HE LOST THE TIP OF HIS RING FINGER.
TRUCK DRIVER WAS MOVING ROLLER BARS ON HIS TRAILER WHEN ROLLERS SHIFTED AND CRUSHED RIGHT LITTLE FINGER TIP. FIRST JOINT WAS CUT OFF. ROLLERS WERE BANDED TOGETHER WITH PLASTIC BANDS & THEY BRO KE, CAUSING THE ROLLERS TO SHIFT.
EMPLOYEE STATED HE WAS IN THE PROCESS OF MOVING SHIELDS ON THE LONGWALL FACE WHEN A SHIELD FAILED TO ADVANCE HE TRAVELED TO THE SHIELD AT WHICH TIME IT STARTED THROUGH ITS SECOND PULL CYCLE. A S IT MOVED, HIS LEFT FOOT SLIPPED AND BECAME CAUGHT BETWEEN THE RELAY BAR AND THE BASE OF THE SHIELD RESULTING IN THE AMPUTATION OF HIS LITTLE TOE AND A LACERATION TO HIS FOOT.
EE WAS WORKING ON LOW HEAD AT WET GRINDING PLANT WHEN HE CUT HIS RIGHT THUMB ABOVE THE LAST KNUCKLE AND BELOW TIP OF THUMB.
VICTIM CLIMBED ONTO PIPE TO CLEAR ACTERIAL FORM THE FEED CHUTE. VICTIM SLIPPED AND FELL APPROX. 9 1/2 FT TO THE FLOOR, LANDING ON THIS SIDE. VICTIM RETURNED TO WORK FOR HIS NEXT SHIFT, HOWEVER ON 9-12-01. HE REPORTED THAT HE COULD NOT COME TO WORK AND WAS INSTRUCTED TO SEE A DAY.
EMPLOYEE WAS CARRYING LARGE STONE. CORNER OF STONE BROKE AWAY. EMPLOYEE LOST HOLD & STONE CAME DOWN OF FOOT.
EE WAS POSITIONED BESIDE SCOOP. WHEN SCOOP OPERATOR LOWERED BUCKET, EE'S LT. KNEE WAS CAUGHT BETWEEN SCOOP BUCKET & TIRE.
INJ WAS USING SLEDGE HAMMER TO FREE CEMENT TRANSFER VALVE THAT WAS STUCK. AS HE SWUNG THE HAMMER HE MISSED THE VALVE & STRAINED HIS SHOULDER. DID NOT BECOME REPORTABLE UNTIL 4/24/02 WHEN HE STARTED TO MISS WORK DUE TO SURGERY Y
AFTER FINISHING CLIPPING A CONVEYER BELT, THE EMPLOYEE STEPPED BACKWARDS AWAY FROM THE CONVEYER. THE EMPLOYEE TWISTED WRONG WHILE WALKING BACK TO HIS LOADER. HE DID NOT FALL OR STUBLE AND THER E WAS NO OTHER APPARENT INJURY.
EMPLOYEE WAS WALKING DOWN AN ESTABLISHED TRAIL ON A SLOPE AND TRIPPED ON A ROCK, TWISTING RIGHT KNEE.
PLACED CONCRETE PAD UNDER STONE BIN. EMPLOYEE WAS TROWELING CONCRETE WHEN A PIECE OF I BEAM SLID DOWN CAT WALK OVER HEAD AND STRUCK EMPLOYEE ON RIGHT SIDE OF HEAD. TWO EMPLOYEES WERE REPAIRING A CONVEYOR.
EE WAS CKEANING UP AROUND THE TIRE BOX. WHEN HE AND ANOTHER EE PICKED UP THE CONTAINER TO EMPTY INTO THE BOBCAT, HE FELT A PULL ON THE LEFT SIDE OF HIS BACK. HE CONTINUED TO WORK HIS NORMAL DA YS, BUT WENT TO THE DR. FOUR DAYS LATER.
THROTTLE STUCK FULL ON, EMPLOYEE COULD NOT TURN ENGINE OFF, JEEP RAN INTO PILLAR, THROWING EMPL-OYEE OUT AND BREAKING HIS LEG ON THE WAY OUT.
EMPLOYEE WAS BOLTING MINE ROOF WHEN HE MOVED THE BOLTER HEAD TO INSTALL SECOND BOLT A PIECE OF DRAW ROCK FELL, STRIKING HIM ON THE HEAD AND BACK.
EE WAS WORKING IN THE NEW CONSTRUCTION AREA OF THE PROCESSING PLANT WHEN HE SLIPPED AND FELL OFFA LADDER. ALTHOUGH ""SPOTTERS"" OR LADDER HOLDERS ARE AVAILABLE, HE OPTED NOT TO TELL ANYONE HE WA S GOING UP THE LADDER. THE DOCTOR SAID HE HAD A ""BRUISED BUTTOCK"" AND WOULD BE O.K. AFTER A SHORT TIME.
EMPLOYEE WAS TRAMMING A BOLT MACHINE. DRAW ROCK FELL FROM BETWEEN THE BOLTS, STRUCK HIM ON THE LEFT SHOULDER.
EE WAS IN THE PROCESS OF INSTALLING A ROOF BOLT IN THE ROOF WHEN A PIECE OF DRAW ROCK FELL AND STRUCK HIM ON TOP OF HIS HARD HAT AND JAMMED HIS NECK.
EE WAS CRAWLING UNDER A TRUCK IN THE SURFACE SHOP TO REPLACE BED LIFTING CYLINDERS WHEN HE FELT WHAT HE THOUGHT WAS A ROCK PUNCTURE TO HIS LEFT KNEE.
EE WAS THROWING A TRACK SWITCH HANDLE TO LET A MOTOR THROUGH. THE HANDLE WAS NOT IN THE LOCKED POSITION WHEN THE MOTOR WENT THROUGH THE SWITCH CAUSING THE HANDLE TO JUMP UP AND HIT HIS KNEE.
THE INJURED WAS STANDING ON TOP OF ROOF BOLTER DRILL HEAD PUTTING IN TUBES OF GLUE AND THE FIRSTSECTION OF BOLT, IN AN OUTBY AREA, WHEN HE STEPPED OFF OF THE DRILL HEAD HIS FOOT SLIPPED ON WET BOTTOM, RESULTING IN AN INJURY TO HIS LEFT KNEE(AREA HAD BEEN PREVIOUSLY BEEN BOLTED).
WHILE TRYING TO BALANCE A ROCK ON ITS EDGE THE ROCK FELL OVER LANDING ON EMPLOYEE'S R. FOOT ANDFRACTURED HIS R. 4TH METATARSAL.
EMPLOYEE WAS STRUCK A GLANCING BLOW WITH A LARGE PIECE OF FALLING ROOF ROCK WHILE STANDING ADJACENT TO A 10SC SHUTTLE CAR, ALONG AN EMPLOYEE WHO WAS WATCHING THE MINER TRAILING CABLE.
STANDING ON LADDER WORKING ON LECTRA HAUL ROCKTRUCK WHEN HE FELL HITTING HIS KNEES, NOTE THIS WAS BEGINNING OF SHIFT, HE WORKED FULL 10 HR SHIFT PLUS TWO HOURS OVERTIME, NEVER REPORTED INCIDEN T TO SUPERVISOR, GATHERED INFO FROM WITNESS.

INJURED WAS STANDING ON LADDER HANGING CABLE. HE FELL OFF LADDER CAUSING A BRUISE TO RIGHT RIBS.
EE WAS PREPARING TO OPERATE A D.C. POWERED TROLLEY MANTRIP. AS HE ATTEMPTED TO PUT THE TROLLEY POLE ON THE TROLLEY WIRE HE CONTACTED THE 600 DC TROLLEY WIRE WITH THE BACK OF HIS LEFT HAND.
INJURED PARTY SAID LOADER MAN PUT A LARGE ROCK ON THE TRUCK, AS THE LOADER WAS BACKING AWAY THE ROCK SHIFTED WHICH IN TURN SHOOK THE DRIVER. HE SAID IT HURT HIS NECK & LT. ARM.
EE WAS GETTING ON #14 GETMAN. EQUIPMENT HAS A SWING TYPE LADDER. OLADDER SUNG UNDER BASKET CAUSING EE TO HYPEREXTEND RIGHT KNEE RESULTING IN ARTHROSCOPY SURGERY ON 10-03-01.
EE WAS WORKING AA A MINER HELPER CUFFING ROOF WHEN THE RIB ROLLED ONTO HIS LEG AND BACK. THE RIB ROLL WAS 7'LX32""WX8"" TO 10"" THICK.
INJURED LOST HIS BALANCE AND TWISTED HIS BACK WHEN HE STEPPED OFF A SMALL LEDGE IN THE MINE FLOOR.
ON AUGUST 30, 2001, EE STATES HE PULLED A MUSCLE IN HIS LOWER BACK. PART OF HIS WORK DUTIES DURING THE SHIFT INCLUDED INSTALLING A SHEVE PIN, THROWING A ROCK FEOM THE WALKWAY INTO THE CHAIN LI NE AND CRAWLING AT THE SHIELD AREA. HE ADDED NO SPECIFIC EVENT COULD BE RELATED TO THE INCIDENT.

EMPLOYEE WAS IN THE PROCESS OF INSTALLING A ROOF BOLT WHEN A PIECE OF DRAW FELL AND STRUCK HIM IN THE BACK.
EE WAS OPERATING THE LEFT SIDE OF A DOUBLE BOOM BOLTER WHEN HIS PARTNER STARTED TO SQUARE UP BOLT TO CONTINUE THE BOLTING PROCESS AT THE BEGINNING OF THE CROSS-CUT. HE WAS ON THE INBY RIGHT CO RNER OF #5 ROOM WHEN THE FRONT
EE WAS DRILLING BOLT HOLES IN ROOF TOP. STEEL GOT STUCK SO HE WAS SPINNING THE HEAD IN AN ATTEMPT TO LOOSEN STEEL, CAUGHT HIS LEFT PINKIE FINGERBETWEEN HEAD OF BOLT MACHINE AND DRILL STEEL.
WHEN THE EMPLOYEE TURNED AIR HOSE ON THE HOSE SLIPPED FROM HIS HAND AND HIT HIM IN THE FACE. THIS KNOCKED HIS SAFETY GLASSES OFF AND STRUCK HIM IN THE RIGHT EYE.
WHILE EXTENDING THE WHEELS ON THE STACKING CONVEYOR, EE INSERTED THE PIN WHICH RESULTED IN PINCHING HIS THUMB.
WHILE SHOVELING IN THE SLOPE, EE FELT PAIN IN HIS LEFT SHOULDER.
EE WAS WORKING ALONE AND IS NOW HOSPITALIZED AND HAS NOT BEEN SPOKEN TO ABOUT THIS INCIDENT. IT IS SPECULATED THAT HE WAS WORK OFF OF A 10 FT EXTENSION LADDER AND FELL APPROX SEVEN FEET AND HI T THE RIGHT BACKSIDE A HIS HEAD. HE WAS TAKEN BYAMBULACNE TO BERKSHIRE MEDICAL CENTER AND WAS SEDATED FOR OBSERVATION. AT SCAN TAKEN WAS NORMAL.
EMPLOYEE WAS FOUND LYING ON FLOOR UNRESPONSIVE, EMPLOYEE BECAME RESPONSIVE APPROX TEN MINUTES LATER AND STATED THAT HE HAD FALLEN DOWN THE SHIPPING STAIRS FROM ABOUT HALF WAY UP. HE STATED THAT ONLY HIS PREVIOUSLY INJURED LEG HAD PAIN.
WAS WORKING WITH SUPERSACKS AND FORKLIFT. TRIED TO STRAIGHTEN OUT THE SUPERSACK WHILE IT WAS ON THE FORKLIFT ARMS. SAID THAT HE WAS PUSHING AND FELT A SHARP PAIN. WORKED THE REST OF THE DAY. SAID THAT HE WAS A LITTLE SORE. ASKED HIM IF HE WANTED TO GO TO THE DR, HE SAID NO, HE WOULD BE OK. HE WENT TO DR ON 9-27. HE PULLED A MUSCLE IN THE GROIN AREA.
EE TRIPPED OVER PIECE OF ANGLE IRON LEFT ON CATWALK, CAUSING A LACERATION AND BRUISE TO RIGHT KNEE.
OPERATOR WHEN DISMOUNTING LOADER HIS HAND MISSED THE HANDRAIL AND HIS FOOT SLIPPED OFF THE LOWER STEP RESULTING IN A BROKEN FOOT.

WAS STANDING ON A SUPPLY CAR HOLDING ONTO SOME 4'X4' PIECES OF 3/4"" PLYWOOD AS A OPERATOR WHO WAS LOADING THE MATERIAL WITH A FORKLIFT BACKED AWAY. AT THIS TIME THE SLOPE HOIST OPERATOR SAW ANOTHER EE STANDING AT THE END OF THE SUPPLY TRIP THAT WAS TO BE DROPPED DOWN THE SLOPE. THINKING THIS EE WAS GOING TO CUT THE SUPPLY TRIP LOOSEFROM THE CAR INJ WAS ON PULLED THE TRAP A FEW IN
DEGREE 6- ARRIVED AT WORK TO START HIS SHIFT. WALKED INTO OLD FOREMANS OFFICE WHER HIS LOCKER IS LOCATED. HIS LEFT FOOT SLIPPED ON SMALL SPILL OF OIL THAT WAS IN FRONT OF THE LOCKER CAUSING HI M TO FALL ON HIS LEFT ELBOW AGAINST FLOOR, TWISTING LEFT KNEE.
EE STATED THAT HE WAS HELPING 2 OTHER EE REMOVE SIDE PLATES AND DEKCS FROM THE #1 RAW COAL SCREEN. WHILE MOVING THIS PIECE TO THE OUTSIDE THE END OF THE MATERIAL CAUGHT ON A HANDRAIL AND TOE P LATE. HE TREID TO PULL THE PLATE AWAY IT SLID OFUF THE TOE PLATE AND CAUGHT THE END OF HIS RIGHT MIDDLE FINGER BETWEEN THE PALTE AND THE CONCRETE FLOOR.

WHILE LOADING CINDER BLOCK INTO THE SCOOP BUCKETAT THE END OF THE TRACK ON THE 012-0 MMU EE FELTA SUDDEN PAIN IN HIS LOWER BACK. HE WORKED UNTIL8/27; HE RETURNED 8/30 MISSING 3 DAYS WORK.
EE WAS UNLOADING 36"" TOP BELT STRUCTURE FROM MINE BUGGY WHEN HE FELT PAIN IN HIS LOWER BACK.

RADIATOR CAP DISLODGED AND EMPLOYEE WAS ""SPRAYED"" WITH HOT COOLANT RESULTING IN 1ST DEGREE BURNS TO BOTH ARMS AND A SMALL 2ND D EGREE BURN TO TOP OF HEAD. EMPLOYEE JUMPED OFF TRUCK AND FRACTURED RIGHT HEEL (BONE CHIP) AND BRUISED LEFT HEEL.

INJURED WAS STRUCK BY A ROCK FALLING OFF OF A SHIELD.
EMPLOYEE WAS ON A PLATFORM UNPLUGGING A DUST LINE. HE FINISHED THE JOB AND CLIMBED DOWN THE LADDER. HE JUMPED FROM SEVERAL RUNGS UP AND LANDED ON A SLEDGE HAMMER THAT HAD BEEN LEFT ON THE FLO OR. HE BROKE HIS RIGHT LEG JUST ABOVE THE ANKLE.
LOADER OPERATOR WAS GREASING BUCKET ARMS AND FELL FROM LOADER TO GROUND. NO DAMAGE TO EQUIPMENT.
AT THE TIME OF THE INDIENT EE WAS USING A CRANE TO LIFT THE HEAD OFF THE ENGINE OF A CAT 793 OFF HIGHWAY TRUCK. AS THE HEAD WAS BEING REAISED IT BECAME STUCK. WHILE EE WAS WORKING TO FREE THE HEAD, THE HEAD BROKE LOOSE UNEXPECTEDLY AND PINCHED HIS HAND AGAINST THE WATER MANIFOLD.
THE EMPLOYEE WAS UNLOADING CRIB BLOCK FROM THE BUCKET OF A UNIHAULER. HE SLIPPED AND FELL OUT OF THE BUCKET AND STRUCK A ROCK ON THE MINE FLOOR. HE BEGAN MISSING TIME ON 8/27/01 DUE TO THIS IN CIDENT.
TWISTED ANKLE WHEN MOVING TO GET AWAY FROM FALLING DRAW ROCK.
TWO PREP PLANT MECHANICS WERE CHANGING A MOTOR BASE. THEY HAD THE MOTOR RAISED USING A 5 TON HOIST & A 1 TON HOIST. AS THEY WERE ALIGNING THE BASE WITH THE MOTOR MOUNTS USING BOLTS TO ALIGN TH E HOLES THE CABLE IN THE 5 TON HOIST SLIPPED APPROXIMATELY 2"". THE MECHANIC'S FINGERS WERE PINCHED BY THE BOLT & BASE.
EMPLOYEE PULLED DRAW ROCK DOWN. HE PUT UP OUTSIDE BOLT, SWUNG OVER TO PUT UP NEXT BOLT. PULLED DOWN DRAW ROCK, PUSHED RIGHT LEG, FELL, TWISTED RIGHT KNEE.
THE DRIVER POSITIONED HIS TRUCK IN THE PIT TO BE LOADER, AND REACHED OVER TO THE DASH TO GET HIS LOAD COUNT REPORT AS THE LOADER WAS APPROACHINGTHE TRUCK TO DUMP THE BUCKET WITHT HE LOADER. THE OPERATOR DROPPED THE BUCKET FULL OF COAL STRIKING THE TOP OF THE BOX CAUSING THE TRUCK TO BOUNCE, RESULTING IN WHIP LASH TO THE TRUCK DRIVER.
WHEN STEPPING DOWN FROM THE LAST LADDER RUNG ON THE TRIPPER TO THE TRIPPER DECK, EE'S RIGHT KNEE BUCKLED AND HE COLLAPSED TO THE FLOOR.
EE WAS WORKING WITH RIB ROLLER AND IT ROLLED OFF ON HIS L LEG AND ARM.
LIFTING HIENTZMAN JACK TO RESET IN PLACE PULLED MUSCLE AND/OR STRAINED SHOULDER.
EE WALKED ON FORM WALL ONTO CONCRETE WALL FALLING APPROX 10 FEET.
EE BRUISED HIS RIBS ON MUCKER SEAT WHEN HE RAN OVER A ROCK IN THE ROAD WHERE HE WAS MOVING ORE. THE INCIDENT WAS NOT PROPERLY REPORTED TO THE MINE AND WAS UNDER INVESTIGATION UNTIL 10-5-01. TH E EE WAS GIVEN WORK RESTRICTIONS BY A DR. AND PRESCRIPTION MEDICATION.
THE EE STATED HE WAS OPERATING THE SHEARER WHEN A JACK SETTER RELEASED SHIELD 179 AND A ROCK FELL OFF THE TOP OF THE SHIELD AND BOUNCED OFF THE TAIL DRIVE ONTO THE SHEARER AND INTO THE FACE LI NE WHERE IT STRUCK HIM ON THE ANKLE.
INJURED WAS DOING MAINTENANCE ON SHOVEL. WHILE LOOSENING RADIATOR CAP THE PRESSURE PUSHED ANIT-FREEZE OUT CAUSING A BURN TO RIGHT ARM.
EMPLOYEE OPENED A FLOOR GRATING ACCESS PANEL TO CHECK A VALVE. LEFT PANEL OFF AND WENT TO CHECK ANOTHER PIECE OF EQUIPMENT. WALKED BACK THROUGH AREA AND FELL INTO THE OPENING. BRUISED RIBS AND ELBOW.

EE WAS PULLING SHIELDS IN TWIN SEAM AREA WHEN A ROCK CAME OFF THE TOP OF THE SHIELD & STRUCK HISRIGHT FOOT.
WAS GETTING READY TO MOUNT DOZER WHEN HE STEPPED UP ON PUSH ARM. HIS FOOTING SLIPPED AND HE FELL ACROSS TILT JACK.
SHUT DOWN CRUSHER AND WERE GOING TO BEGIN TEARING DOWN FOR A MOVE THE NEXT MORNING. HE OPENED THE ELECTRICAL BOX ON THE PRIMARY PLANT AND WAS GOING TO BEGIN UNHOOKING THE ELECTRICAL CONNECTION S BUT THE BOX WAS STILL LIVE. HE WAS THROWN FROM THE BOX, BUT SUSTAINED SEVERE BURNS TO LEFT HAND AND ARM.
EE WAS CLEANING & DUSTING THE BELTS. HE WAS CARRYING A BAG OF ROCK DUST UNDER HIS ARM, HE TRIED TO OPEN AN AIRLOCK DOOR. THE AIR PRESSURE COMBINED WITH THE SLICK BOTTOM CAUSED HIM TO FALL. HE LANDED ON A CINDER BLOCK AND AGAINST A HAND RAIL CUASING THE INJURY.
WAS STANDING BESIDE THE FULL FACE MINER WHEN A PIECE OF ROOF COAL 18"" X 27"" X 2"" FELL OUT FROM BETWEEN THE STRAPS AND HIT HIM ON THE HARD HAT.

WHILE PUSHING SLOTS WITH HIS DOZER, THE INJURED BACKED OVER A HUMP AND THE DOZER SLAMMED DOWN JARRING & STRAINING HIS TAIL BONE.
EE WAS MOVING A NEW FILE CABINET, 4 DRAWER, AND STRAINED HIS BACK.
8-23-01 EE ASKED FOR DAY OFF TO GET HERNIA SURGERY-LATE SOAP IT HAPPENED AT WORK 3 TO 4 WEEKS PRIOR TO ACCIDENT DATE.
EMPLOYEE STATED WHILE SWINGING SLEDGE HAMMER FELT PAIN IN BACK. EMPLOYEE HAS HAD PRIOR BLOCK IN BACK BEFORE. EMPLOYEE STARTED MISSING WORK ON8-27-01.
WAS TRAMMING FROM RECLAMATION SITE OF WORK ON ANOTHER COMPANY PERMIT WHEN HE TURNED ABRUPTLY AND LOST CONTROL. WENT INTO CREEK, BUMPING HIS HEAD CAUSING A BUMP ON HIS HEAD.
EE WAS WALKING UNDER PLANT, LOST FOOTING CAUSING HIM TO TWIST RIGHT KNEE AND FELL ON LEFT BACK SIDE.
THE WATERLINE BROKE, EE WORKED IN WET BOOTS FOR SEVERAL DAYS AFTER THAT INCIDENT.
EE WAS PULLING SLACK ROPE WHEN HE FELT PAIN IN LOW BACK.
INJURED WAS STRUCK BY A ROCK FALLING BETWEEN SHIELDS.
GETTING OUT OF TRUCK AND FELL ON GROUND AND HURT RIGHT KNEE AND RIGHT ANKLE.
WHILE PULLING MINER CABLE EE FELT PAIN IN HIS ARM. IT CONTINUED TO BOTHER HIM & HE SOUGHT MEDICAL ATTENTION ON 8/30/01. IT WAS DETERMINED HE HADRUPTURED HIS BICEP MUSCLE & WOULD NEED SURGERY.
ALLEGED INCIDENT WAS TO HAVE OCCURRED WHILE THIS EE WAS HELPING TO UNLOAD ROCKDUST BAGS FROM SUPPLY TRAILER. HE SAID HE SLIPPED ON A PIECE OF COAL AND STRUCK HIS LEG AGAINS SUPPLY CAR. CAUSE O F INJURY WAS AWARENESS. COMPLIANCE WITH RULES/REGS WAS NOT A FACTOR, MINER ATTITUDE WAS A FACTOR, MINING EQUIP/SYSTEMS COULD BE A FACTOR, SAFETY RELATED ITEMS NOT A FACTOR.

EE SWUNG BOOM OUT TO SPOT A PIN. WHEN HE SWUNG BOOM BACK, THE CANOPY HUNG ON ATRS SYSTEM. THE ATRS SYSTEM WAS MOVED BY THE OTHER OPERATOR AND THAT CAUSED THE CANOPY TO DROP APPROX 6"" TO 8"", HI TTING HIM ON TOP OF HIS HEAD.
REMOTE OPERATOR MOVING TRAIN FROM CRUSHER TO QUARRY TRAVELED OVER A FACING SWITCH POINT THAT WAS NOT SECURED AND DERAILED 2 EMPTIES.OUTSIDE CONTRACTOR WAS WORKING ON SWITCH AND DID NOT SECURE BEFORE GIVING PERMISSION TO OPERATOR TO TRAVEL OVER IT.
LIGHTENING STRUCK DOZER OR CLOSE TO IT. HE HAD NUMBNESS IN WRIST AND HANDS.
WHILE APPROACHING A DRILL TO BE FUELED, THE EMPLOYEE FAILED TO DETECT A LARGE SURFACE CRACK. HE STEPPED INTO THE CRACK AND FELL. HE ATTEMPTED TO BREAK HIS FALL WITH HIS HAND, THE IMPACT CAUSED THE INJURY.
WAS BENT OVER A SCOOP HOOKING UP THE BATTERIES WHEN HE FELT A PAIN IN HIS BACK.
EE WAS CHANGING BIT ON SLOT DRILL. THE 10' STEEL FELL OUT OF HOLDER, TIPPING OVER AND HITTING EE ON HARD HAT, UPPER BACK AND LEFT ELBOW, CAUSING CONTUSION TO UPPER BACK. Y

EE WAS ASSISTING IN LAYING PIPE IN A 3-4 FOOT TRENCH. THE TRENCH COLLAPSED AND BURIED EE UP TO BELOW HIS HIPS.
DRAW ROCK FELL OUT FROM BETWEEN ROOF SUPPORT AND STRUCK THE EMPLOYEE ON HIS HEAD AND NECK.
HE WAS ATTEMPTING TO TURN OFF POWER SOURCE TO A WATER PUMP. WHILE PULLING THE FUSE SAFETY SWITCH HANDLE TO THE OFF POSITION, THE ARC SHIELD EXPLODED AND BLEW THE ELECTRIC BOX OPEN RESULTING IN SERIOUS INJURY.
EE WAS PREPARING TO MOVE SECTION WATER PUMP. HE WAS NOT AWARE THE PUMP HAD NOT BEEN CIRCULATING AND THE WATER INSIDE WAS VERY HOT. HE UNCOUPLED WATER LINE, HOT WATER SPRAYED ONTO HIS BODY FROM NECK TO LEGS. INJURIES RESULT IN 2ND & 3RD DEGREE BURNS.
SLIPPED ON STAIRS COMING OUT OF #3 COOLER PIT. LANDED ON LEFT KNEE.
EMPLOYEE WAS REVERSING ROCK TRUCK DOWN INCLINE TO DUMP AREA. WHEN DIRECTED TO STOP BY DOZER OPERATOR ATTENDING THE DUMP, HE FAILED TO DO SO. EMPLOYEE BACKED TRUCK INTO A HOLE CAUSING FRONT WHE ELS TO LEAVE ROADWAY. EMPLOYEE PANICKED AND JUMPED FROM TRUCK CAUSING INJURY TO LEFT LEG.
EE STATED HE WAS STRUCK BY A MRS CABLE IN THE CHEST KNOCKING HIM TO THE MINE FLOOR CAUSING A RIGHT KNEE SPRAIN AND BRUISE. THE MRS CABLE CAUGHT ON A PIECE OF ROCK THAT WAS ON THE LOADED COAL H AULER. THE COAL HAULER WAS MOVING AND PULLING THE CABLE OUT OF THE HANGERS AND STRIKING THE EE.
EE STATED WHILE SETTING TIMBERS ON PILLAR LINE HE FELT PAIN IN HIS GROIN
WAS GETTING A TEST AND ACCIDENTALLY STUCK HAND IN ROTER IN TOP OF MICRO SIZER WHICH CUT TIP OF MIDDLE FINGER ON RIGHT HAND.
EE WAS HAULING REFUSE MATERAIL TO STACKER BELT ON IN POUNDMENT. HE WAS DUMPING MATERIAL, IT STUCK IN BED AS IT WAS RAISED. THE CMB OF TRUCK WAS LIFTED UP AND IT SWIVLED ONTO ITS SIDE. HE ALLEG ED IT STRAINED HIS BACK. BED OF TRUCK DID NOT TURN OVER.
THREE MEN WERE ATTEMPTING TO SLIDE A NEW CONVEYOR MOTOR FROM A SCOOP BUCKET WHEN ONE FELT PAIN IN HIS BACK.
EE PICKED UP THE ENGINE LIFTING BRACKET AND FELT PAIN IN HIS LOWER BACK. LOST TIME INJURY 8-22-01.
EE WAS USING A WRENCH TO LOOSEN LUG NUTS ON A SHUTTLE CAR WHEEL AND FELT PAIN IN HIS RIGHT SHOULDER.
WHILE ATTEMPTING TO ROTATE A TIRE, THE TIRE EXPLODED BRUISING THE SHOULDER AND ARM AND PUTTING DEBRIS IN EYES FACE SHOULDER AND ARMS.
EMPLOYEE WAS INSTALLING A 12' CABLE BOLT IN DRILL HOLE WHEN IT BECAME STUCK IN THE LAST 2' OF THE HOLE. AS HE LIFTED THE BOLT WITH THE DRILL HEAD IT SPRANG BACK AND STRUCK HIM ON THE LEFT FORE ARM APPROX 3"" ABOVE HIS WRIST. THIS RESULTED IN A FRACTURE OF THE ULNAR BONE AT THAT LOCATION.
EE FELT DISCOMFORT IN HIS SHOULDER DURING THE SHIFT. THERE WAS NO SPECIFIC TIME. HE SAW A SPECIALIST IN JAN 2002 WHO RECOMMENDED AN ORTHOSCOPIC PROCEDURE TO REPAIR WHAT HE DIAGNOSED AS A ROTAT OR CUFF INJURY. THE PROCEDURE REVEALED AN OLD FRACTURE AND CHIPS OF BONE THAT WERE CAUSING THE PAIN. THE PROCEDURE BECAME MORE COMPLICATED RESULTING IN LOST WORK DAYS STARTING 3/6/02.
WAS MAKING A BELT MOVE AND AS HE TURNED, HE FELT PAIN IN HIS KNEE.
DURING KILN SHUTDOWN, EE WAS REMOVING STEEL DOORON ELECTROSTATIC PRECIPITATOR DUCT IN ORDER TO INSPECT AND REPAIR DUCT WHEN HE DROPPED THE STEELDOOR ON HIS LEFT FOOT. X-RAY NEGATIVE FOR FRACTU RE BUT HIS FOOT WAS BADLY SWOLLEN. HE LOST 1 DAYOF WORK.
RETRACKING BEAMS USING HAMMER, HAMMER MISSED BARAND HIT FINGERS ON RIGHT HAND.
EMPLOYEE WAS ROOF BOLTING AND GOT HIS LEFT HAND CAUGHT BETWEEN 2 PIECES OF DRILL STEEL.
THE INJURED WAS LOCATED IN THE #3 ENTRY AT THE FEEDER ON THE #1 SECTION. HE WAS TURNING A BOLT ON A SPEED REDUCER WITH A 3/4"" RATCHET DRIVE, A 1 7/8"" SOCKET AND 10"" EXTENSION. THE SOCKET CAME LOOSE CAUSING A MUSCLE STRAIN IN THE BOTTOM RIGHT SIDE OF HIS BACK. WE WERE NOT NOTIFIED UNTIL 8/28/01. THIS IS A DEGREE 3 CLASS 10 ACCIDENT.
EE WAS CHECKING HIS LOAD ON A DUMP TRK & SLIPPED& FELL BACKWARDS (LADDER ON THE TRAILER)
EMPLOYEE HAD COMPLETED WORK IN THE BELT ENTRY. AS HE WAS PASSING THROUGH A MANDOOR IT CLOSED ON HIS LEFT HAND AND FINGERS. HIS FINGERS WERE BRUISED AND A FINGERNAIL WAS REMOVED.
