#1 ENTRY OF MINE CUT THROUGH INTO 32"" AUGER HOLEON LEFT RIB. AUGER HOLE WORK NUT PROPERLY PLOTTED ON MINE MAP. HOLE CONTAINED WATER, WHICH HAD TO BE PUMPED.
THE HOIST WAS LOWERING IN THE AUTOMATIC MODE. THE CAGE HAD AN EMPTY CAR ON IT WHEN A BRAKE SHOE SET AT APPROX. 50 FEET ABOVE THE COAL SEAM. ONLY ONE SHOE CAUGHT, CAUSING THE CAGE TO BEND ITS G UIDE ROLLERS.

THE IGNITION HAPPENED WHILE MINING WAS IN PROGRESS FROM THE HEADGATE TO THE TAILGATE AT 126 SHIELD. A JOY 7LS SHEARER WAS BEING USED. THE FLAME LASTED BETWEEN EIGHT AND TEN SECONDS AND WAS EXTINGUISHED BY WATER FROM THE TAIL DRUM SPRAYS. NO INJURIES.

FALL WAS ON SECTION TRACK AND HIGH VOLTAGE CABLE CAUSING SEVERAL HOURS OF PRODUCTION LOSS. APPROACHES ARE SUPPORTED AND DANGERED OFF. FALL CAVITY 8'HIGH, 17'LONG AND 17'WIDE.

INJURED WAS HELPING TO SET A DUKE. HE HAD HIS RIGHT HAND ON THE HANDLE OF THE DUKE WHEN A CRIB BLOCK FELL OFF THE TOP OF THE DUKE STRIKING HIS LITTLE FINGER OF HIS RIGHT HAND CAUSING AN AMPUTA TION FIRST JOINT.

HE THOUGHT THE BOLT WAS GOING TO SET UP AND HE USED HIS RIGHT HAND TO MOVE THE BOLT OVER FOR ALIGNMENT. AT THE SAME TIME HE APPLIED UP PRESSURE TO THE DRILL POD AND HIS RIGHT MIDDLE FINGER WAS CAUGHT BETWEEN THE PLATE AND ROOF. THE TIP OF FINGER BEHIND NAIL WAS SEVERED.
EMPLOYEE WAS INSTALLING TRUSS BOLTS USING A HYDRAULIC GUN. AFTER THE GUN PRESSES THE TRUSS TIGHT IT RELEASES TO TIGHTEN THE BOLT. HE PLACED HIS LEFT HAND ON THE GUN AND WHEN IT RETRACTED IT AM PUTATED THE FIRST JOINT OF HIS LITTLE FINGER.

WHILE INSTALLING A BRASS SADDLE BLOCK BUSHING IT BECAME LODGED IN THE SADDLE BLOCK. EE ATTEMPTED TO STRAIGHTEN THE BUSHING IT BOTTOMED OUT CATCHING EE'F FINGER BETWEEN THE BUSHING AND THE SADD LE BLOCK.

EMPLOYEE WAS CHECKING THE OIL ON 990 CAT LOADER,STEPPED BACK TO CLOSE DOOR, EMPLOYEE LOST HIS FOOTING AND FELL APPROX 4 1/2 FT ON BUTTOCKS AND HURT HIS BACK.
EMPLOYEE WAS IN PROCESS OF STEPPING OFF OF 777-A HAUL TRUCK LADDER. WHILE STEPPING OFF SPRAINED LEFT ANKLE WHEN CONTACT WAS MADE WITH GROUND.
EE WAS PULLING A TRIP OF ROCK CARS OUT OF THE MINE. CARS WERE LOADED HIGH AND HITTING THE ROOF. EE STOPPED TO MOVE ROCK AROUND IN A CAR. A ROCK SLID DOWN AND CAUGHT EE RIGHT THUMB AGAINST SIDE OF WAGON.
HE WAS DRIVING A MOTOR VEHICL AND HAS A COLLISION. HE SUSTAINED A LACERATION ON HIS LEG.
THE DUMP TRUCK WAS BEING LOADED WITH SHOT ROCK &WHEN IT TURNED THE BUCKET OVER TO DUMP INTO THE TRUCK BED THE ROCK BOUNCED OFF THE CAB SHIELD & ROLLED OVER ONTO THE CAB OF THE TRUCK. AS THE RO CK FELL THE CAB ROOF BEGAN TO CAVE SLIGHTLY CAUSING THE DRIVER TO QUICKLY MOVE TO THE LEFT TO AVOID BEING HARMED, POSSIBLY AGGRAVATING A PREVIOUS INJURY (HE REPORTED NUMBNESS A FEW DAYS LATER
EMPLOYEE STATED THAT HE STOPPED #43 JEEP AT 1-WEST ELECTRIC ON THE 3-NORTH SIDE, TO THROW THE ELECTRIC SWITCH, HE SET THE BRAKE AND STARTED TOSTEP OUT OF THE JEEP WITH HIS RIGHT FOOT FIRST ALONG SIDE OF THE JEEP, THE JEEP STARTED TO DRIFT TOWARD 3-NORTH HIS RIGHT FOOT CAUGHT IN THE WHEEL OF THE OUTBY SIDE OF THE JEEP.

EE WAS WELDING ON DOZER ARM. HIS BACK BECAME INCREASINGLY SORE WITH NUMBNESS IN RIGHT FOOT. HE WENT TO E.R. ON 9/30 & REPORTED INCIDENT TO MANAGEMENT ON 10/1. HE WAS TAKEN TO HOSPITAL WHERE X- RAYS, CAT SCAN & MRI WERE PERFORMED. HE WAS DIAGNOSED WITH A HERNIATED DISC WITH NERVE COMPRESSION. SURGERY WAS PERFORMED 10/2/01. HE WILL BE UNABLE TO RETURN TO WORK FOR 12-16 WEEKS.
EE HAS HAD OTHER INCIDENTS OF BACK PROBLEMS. IT IS UNKNOWN WHEN OR WHAT AGGREAVATED HIS BACK CONDITION THIS TIME.
WHILE EE WAS TIGHTENING UP A TUSSBOLT THE BULLETON THE OTHER END GAVE WAY CAUSING THE TRUSS TO WHIPLASH STRIKING HIS LEFT KNEE.
EMPLOYEE HAD BEEN WELDING AND USING A GRINDER AND A FOREIGN OBJECT GOT INTO LEFT EYE.
HE WAS WORKING WITH A DUMP TRAILER AND A PIECE OF METAL FELL FROM THE DUMP, HITTING HIS LEFT SHOULDER. PUTTING DUMP TRAILER ON THE HITCH.
EMPLOYEE WAS MOVING BELT LINE (PULLING) AND FELT PAIN IN HIS BACK TO HIS FOOT.
EE CALLED GC ON SEPT 28 2001 SAID SHE HAS BEEN TO A DR. BECAUSE OF BACK PAIN. DR SAID SHE PROBABLY HAD A COMPRESSED DISK IN NECK CAUSED BY THE REPETATIVE BOUNCING OF OPERATING A SHUTTLE CAR, S HE DID NOT FEEL THERE WAS A SPECIFIC INCIDENT CAUSING HER PROBLEMS.

WHEN THE LOADOUT OPERATOR RETURNED FROM A SAFETY CLASS & TOOK OVER LOADING TRUCKS, HE DID NOT KNOW THE MECHANICS WERE WORKING ON THE AIR SLIDE. HE SWITCHED THE KNOB TO LOAD OUT OF #2.
A DRILL ROD CAME LOOSE FROM THE ROD LOCK AT THE TOP OF THE CAROUSEL. THE DRILLER WHO WAS INJUREDCLIMBED UP THE TOWER TO PUSH THE DRILL ROD INTO POSITION. HIS HELPER LOWERED THE TOP HEAD ONTO THE DRILL ROD TO PUT IT INTO THE CAROUSEL. WHEN LOWERING, THE DRILLER ON THE TOWER HAD HIS HAND IN THE PATH OF A TRAVELING BLOCK & HIS HAND WAS CAUGHT BETWEEN THE BLOCK & THE TOWER.

EMPLOYEE WAS CUTTING OFF A ZIP TIE FROM A METAL LINE WITH A POCKET KNIFE. THE KNIFE SLIPPED & HE STABBED HIS LEFT FOREARM JUST ABOVE THE WRIST. WENT TO WHITESBURG HOSPITAL ER & RECEIVED 4 STAP LES.
EE HAD JUST COMPLETED DRILLING HIS HOLE FOR HIS INSIDE BOLT. HE WAS HOLDING A 4' DRILL STEEL WRENCH IN HIS LEFT HAND WITH IT RESTING ON THE METATARSAL GUARD OF HIS LEFT FOOT. AS HE LOWERED THE DRILL POT IT CAUGHT THE WRENCH CAUSING IT TO CRUSH THE METATARSAL GUARD. HE WAS TRANSPORTED TO HOSPITAL WHERE X-RAYS REVEALED FRACTURES TO 2ND AND 3RD METATARSALS OF HIS LEFT FOOT.
EE WAS WAITING TO DUMP LOAD OF ROCK WHEN ANOTHER LOADER BACKED INTO HIS LOADER.
EE WAS CONDUCTING CONT. & RESISTO TEST ON LEAD OUT BINE. WHILE CLIMBING DOWN HIS LEFT FOOT SLIPPED FROM LADDER RUNG CAUSING HIS ANKLE SPRAIN.
EE SLIPPED AFTER GETTING OUT OF SHOWER, STRIKINGLEFT FOOT ON LOCKER. HE DID NOT MISS ANY DAYS. HE THEN AGGRAVATED THE SAME INJURY AND STARTED MISSING TIME 12/28/01.

EMPLOYEE WAS HELPING REMOVE A TIRE FROM A SHUTTLE CAR. AS THE TIRE CAME OFF THE WHEEL UNIT IT SLIPPED AND CAUGHT THE EMPLOYEE'S LEFT HAND BETWEEN TIRE AND FRAME OF SHUTTLE CAR.

EMP WAS HANGING VENT LINE FROM A 2 YD LOADER AS WAS HIS PARTNER ON THE OTHER SIDE OF STOPR. PARTNER HAD KINK IN VENT LINE & ALL AIR RUSHED TO THE OTHER SIDE AND VENT LINE KNOCKED HIM OFF LOADE R TO THE SILL. HE LANDED ON THE LEFT SIDE OF HIS BACK & FRACTURED 2 RIBS.
WHILE ATTEMPTING TO SHUT THE ENGINE OFF A 6X8"" HIGH PRESSURE PUMP, THE DISCHARGE HOSE BUSTED WHILE DRAINING BACK INTO THE PUMP CAUSING THE EE TO GET DRENCHED WITH HOT WATER THAT WAS IN THE TUR BINE THAT WAS CREATING FRICTION.
EE WAS CHECKING THE VOLTAGE ON A 1000 VOLT MINING MACHINE AND THE VOLT METER HE WAS CHECKING WITH MALFUNCTIONED CAUSING HIS HAND TO BE BURNED.
EE REPORTED ON 10/15/01 THAT HE HAD INJURED HIMSELF ON 9/13 BY USING A ROPE AND PULLEY TO LIFT APIECE OF METAL TO THE TOP OF THE RAW FEED BINS. THE EE WAS BUILDING A STOPPING ON 9/17/01 & PULLED GROIN MUSCLE. THE COMPANY WAS INFORMED ON 10/14/01 THE EE SAW A SPECIALIST ON 10/15/01 & A HERNIA OPERATION IS NEEDED AT A LATER DATE
EE WAS MOVING A SET OF TORCHES THAT WERE MOUNTEDON A WHEELED DOLLY. AS HE BACKED UP HE STEPPED INTO A DRAIN THAT IS 16"" SQUARE & ~1/2"" DEEP AND FELL WITH THE TORCHES FALLING ON HIM.

INJURED WAS STEPPING FROM THE BOTTOM STEP. PUT HIS LEFT FOOT DOWN AND TURNED TO HIS LEFT. HIS FOOT DID NOT PIVOT AND HE FELT PAIN IN HIS LEFT KNEE, AS HE TWISTED.

EMPLOYEE WAS EXITING THE REAR OF THE FORD F450 SERVICE TRUCK. THE TRUCK WAS EQUIPPED WITH A CABLE STEP MOUNTED TO THE REAR BUMPER. AS HE STEPPED ON THE CABLE STEP, THE WELD CONNECTING THE CABL E AND RETAINING NUT BROKE. AS THE STEP GAVE WAY THIS CAUSED A TWISTING MOTION TO HIS BACK RESULTING IN A BACK MUSCLE STRAIN. HE WAS DIAGNOSED WITH MUSCLE SPASMS AND RESTRICTED FROM WORK.

EE WAS READING INTERNAL MEMOS AT HIS DESK. HE TURNED LEFT TO WALK AWAY CAUGHT THE INSIDE OF HIS RIGHT FOOT ON HIS OFFICE CHAIR CAUSING HIS RIGHT KNEE TO TWIST. EE WORKED WITH NO RESTRICTIONS U NTIL 10-2-01 WHEN HE WAS PLACED ON NO WORK STATUS AS PHYSICAN IS SCEDULING SURGERY.
EE WAS TORQUING DOWN BOLT ON 920 CATERPILLAR LOADER, LOST FOOTING AND FELL BACK. EE S HARD HAT CAME OFF AS HE FELL BACKWARDS. HIT BACK OF HEAD AND SPRANG HIS NECK.
