AT THE INTERSECTION OF 40N AND 6E IN KEASTON THE NORTHSIDE OF MINE A ROOF FALL OCCURRED. THE FALL WAS UNWITNESSED AND NO INJURIEDS RESULTED. THE FALL WAS ABOUT 50 FEET IN DIAMETER AND ABOUT 15 FEET HIGH. THIS WAS A ABOVE THE ANCHORAGE ZONE.IT WAS DISCOVERED BY EE AS HE WAS TRAVELING TO TEMCO PHONE.
A SUSPECTED METHANE GAS IGNITION OCCURRED NEAR THE TAIL ON THE 8E LONGWALL SECTION. NO INJURIES OR PROPERTY DAMAGE RESULTED FROM THIS OCCURRENCE.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #4 ENTRY APPROX. 7000 FEET FROM THE ACTIVE FACE NEXT SPAD 11030. THE FALL MEASURED 60' IN LENGTH 19' WIDE AND 5 1/2' HIGH. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERED OFF. THE AREA WILL BE REHAB.
AN ELECTRICAL PROBLEM WAS DISCOVERED WITH THE EMERGENCY HOIST. MINERS WERE EVACUATED BY MAIN HOIST. ELECTRICIANS REPAIRED PROBLEM AND MINERS WENT BACK UNDERGROUND. COMMUNICATED WITH MSHA OUT O F VINCENNES ON THIS DAY ABOUT THE PROBLEM.
A ROOF FALL OCCURRED ON THE IRIW-2L, AT X CUT 15, ENTRY 5, THIS FALL WAS LOCATED IN AN INTERSECTION ON THE TRAVELWAY. THE FALL WAS 24' LONG 30' WIDE AND 8' HIGH. THE CAUSE OF THE FALL WAS WEAK LAMINATED SANDSTONE.
1 RIGHT - 5 SOUTH LONGWALL HEADGATE - BELT ENTRY INTERSECTION AT 69+80. ROOF FELL OUT AROUND 12' CABLE BOLTS. ROOF FALL WAS 10'HIGH, 18'WIDE, AND 20'LONG. AREA WAS INITIALLY MINED IN FEBRUARY 2000. 850 FEET OF COVER.
AN UNPLANNED ROOF FALL OCCURRED ON 7/30/01. DISTANCE FROM ACTIVE FACE IS 2400'. THERE WERE NO PERSONAL INJURIES AND NO EQUIPMENT INVOLVED. SIZE OF FALL IS APPROXIMATELY 20' X 20' X 8'. ALL ENT RANCES TO FALL HAVE BEEN TIMBERED OFF.
A FALL OF ROOF OCCURRED IN THE WEST MAINS AT X CUT AS IN THE #5 ENTRY. THE FALL WAS APPROX. 20'LX20'WIDEX7-8' THICK. THE AREA WAS SUPPORTED WITH 6FT FULLY GROUTED BOLTS AND TRUSS BOLTED. THE A PPROX. OF DEVELOPMENT WAS 9-28-98 AND THE DISTANCE FROM THE SECTION WAS 10,500 FT ADEQUATE WARNING WAS GIVEN.
A NON INJURY ROOF FALL WAS LOCATED BY THE MINE FIRE BOSS. THE FALL WAS AT SPAD #2697 IN THE #2 ENTRY. THE FALL WAS 20' W X 50'L X 5' THICK.
ELECTRICAL WIRE WENT TO GROUND CAUSING HOIST TO STOP OPERATING. PROBLEM WAS DIAGNOSED AND REPAIRED AND HOIST WAS BACK IN OPERATION IN 40 MINUTES.
TWO DAY SHIFT MAINTENANCE EES WITNESSED A VISIBLE FLAME BETWEEN THE SKIRT BOARD & THE CONVEYOR BELT AT THE #1 CRUSHER. THE FLAME WAS CONTRIBUTED TO AN UNPLANNED IGNITION OF COAL DUST IN THE HO PPER.
A ROOF FALL HAS OCCURRED IN THE 12TH SUBMAIN EAST AT CROSSCUT 16 BETWEEN NO.6 AND NO.7 ENTRY. THE DIMENSIONS OF THE FALL ARE 60'LONG, 17'WIDE AND 7'HIGH.

INITIAL CAVE FROM LONGWALL STARTUP CAUSED AIR BLAST THAT DAMAGED SEVERAL STOPPINGS IN RETURN AREA OF LONGWALL. VENTILATION WAS CHANGED DUE TO THE AIR BLAST. 77,000 CFM WAS STILL MAINTAINED ALO NG FACE AREA. STOPPINGS WERE REPAIRED, VENTILATION RESTORED, MINING RESUMED. NO ACCIDENTS OF INJURIES.

A FLASH FLOOD OCCURRED AT THE GARDEN PORTAL WHICH CAUSED ELECTRICAL DAMAGE TO THE AUTOMATIC ELEVATOR. THE ELECTRICAL MOTOR WAS SENT TO AN ELECVTRICAL FACILITY TO BE CLEANED AND THE ELECTRICAL CONTROLS WERE CLEANED AND NECESSARY REPAIRS MADE.

THE AREA HAD BEEN SUP PORTED WITH 5' CONVENTIONAL TYPE ROOF BOLTS. THEFALL MEASURED 25' BY 16' BY 5.5'. THE UNIT WAS IDLE AND THE FALL WAS FOUND BY THE EXAMINER ON THE LAST SHIFT OF THE WEEK. FALL WILL BE CLEANED.

THE AREA WAS MINED ON 8/16/99, THE FALL WAS 6'HIGH, 16'WIDE, 15'LONG. SUPPORTED BY 6' FULLY GROUNDED RESIN BOLTS, CRIBS, CROSSBAKS.

LIGHTINING STRUCK AND CAUGHT THE GRASS ON FIRE IN AREA E NEAR 6E RAMP AND POND/15E JOURDATION, OFIRE DEPT RESPONDED AS WELL AS FIRE TRUCK AND BROUGHT THE FIRE UNDER CONTROL AROUND 1:00 AM THUR SDAY.
A FIRE WAS STARTED BY LIGHTNING IN A RECLAIMED AREA OF THE MINE. THE FIRE WAS APPROXIMATELY 5 MILES FROM THE SHOP AND 16 MILES AWAY FROM WHEREMINING ACTIVITIES WERE TAKING PLACE.
BETWEEN 1:00 AM AND 6:00 AM A ROOF FALL OCCURRED IN THE #1 ENTRY 21 BREAKS OUT BY WORKING FACE MEASURING 3.5 FT THICK 18 FT WIDE AND 70 FT LONG. 42"" RESIN BOLTS WERE BEING USED. AREA WAS MINE D LAST OF MAY 2001.
ROOF FALL OCCURRED IN INTERSECTION AT #4 HEADINGSPAD #2463. FALL OCCURRED AFTER STARTING LEFT X-CUT - RIGHTHAND CROSSCUT WAS ~2 CUTS DEEP. FALL MEASURED 16' LONG, 18' WIDE, 8' HIGH. CONDITIONS CONTRIBUTING TO FALL WERE FAULT AREA: HIGH ANGLESLIP. VENTILATION NOT DISTURBED. EQUIPMENT NOT INVOLVED. NO INJURY. FALL WILL NOT BE CLEANED UP

AT 12:45 AM A ROOF FALL WAS DISCOVERED IN #3 ENTRY OF 4 LEFT SECTION MMU 001-0 AT X-CUT #30, THE ROOF FALL MEASURED 17 1/2 FT IN WIDTH, 18 FT IN LENGTH AND JUST OVER 7 FT HIGH. THE ROOF FALL W AS JUST ABOVE THE ANCHORAGE ZONE. THE ROOF WAS PREVIOUSLY BOLTED WITH 7 FT FULLY GROUTED RESIN BOLTS ON 5 FT CENTERS.
UNINTENTIONAL ROOF FALL OCCURRED ON 009 SECTION PANEL #3 ENTRY FROM THE 21ST X-CUT TO THE 23RD X-CUT. AREA BOLTED WITH 60"" POINT ANCHOR WITH 10'ROPE BOLTS INSTALLED IN INTERSECTION.
A FIRE OCCURRED ON THE OIL COOLER FOR #4 TURBINEGENERATOR IN THE MONO POWER HOUSE. THE FIRE OCCURRED WHEN AN EMPLOYEE LOOSENED AN ""O"" RING TO REPLACE THE H2O TUBES. THE SYSTEM WAS UNDER 25 PSI PRESSURE CAUSING OIL TO SPRAY ON A 600# STEAM LINE WITH SURFACE TEMPERATURE OF APPROXIMATELY 750F. THIS CAUSED THE OIL TO IGNITE STARTING THE FIRE.
TOP WAS CHANGING INTO SANDSTONE. TRANSITION AREA HAD A HORSEBACK AND SOFT SHELL. THE ENTRY WAS STOPPED AND CRIBBED OFF. IT FELL ONE WEEK LATER.
CONTINUATION OF ROOF FALL @ SPAD 99 IN RETURN ENTRY. WAS IN INTERSECTION OF A PREVIOUSLY CRIBBED AREA.
A ROOF FALL WAS DISCOVERED ON THE TRACK ON THE PRESHIFT OT 001 SECTION. THE FALL WAS APPROX. 22' L X 18' W X 9' H. THE AREA IS TOTAL CRIBBED & DANGERED OFF & TRACK RELOCATED IN THE ADJACENT EN TRY. NO EQUIPMENT, EES NOR VENTILATION WAS AFFECTED.

TOP BROKE LOOSE OVER 4 STEELS SETS AT 97XC EAST MAINS AND BOW DOWN THE STEEL SETS WHICH RESTRICTED HAULAGE. CONTINUATION OF OLD FALL.
A FALL OF THE ROOF OCCURRED ON #3 UNIT, #3 ENTRYX-CUT #11 OF THE SOUTH APPROX 20' BY 12' BY 8' HIGH IN A 4 WAY INTERSECTION, BOLTED WITH 6' POINT ANCHOR BOLTS. NOT TO BE CLEANED UP. CAUSE APPE ARS TO BE A SLIP.
THE #1 ELEVATOR WAS TAKEN OUT OF SERVICE ON 7:40 AM, 7/21/01 WHEN IT WAS STOPPING SHORT OF THE TOP LANDING AND CONTINUING ON INSPECTION SPEED AND LEVELING UNEVEN AT THE TOP. REPAIR WAS CALLED OUT AND DURING TROUBLE SHOOTING FOUND THE ELEVATOR OUT OF ALIGNMENT & HOISTING DOOR FRAME LOOSE.THE CAR ROLLERS WERE ADJUSTED TO ALIGNMENT & HOIST DOOR FRAME BOLTED DOWN. BACK IN SERVICE 11:45

A ROOF FALL WAS FOUND DURING A PRE-SHIFT EXAMINATION APPROX 3 BREAKS IN THE MIDDLE FORK PORTAL, #1 OUTBY. THE FALL WAS CLEANED UP CRIBBS AND BEAMS & JACKS ADDED TO SUPPORT THE AREA. FALL WAS A PPROX. 12' LONG X 18' WIDE X 6"" THICK.

AFTER RETURNING FROM OUR VACATION PERIOD, WE DISCOVERED A FALL IN THE #7 ENTRY OF 1ST NORTHWEST MAINS BETWEEN BREAKS #89 AND 91. IT MEASURED 18' W X 90' L X 5' THICK. NO EQUIPMENT OR PERSONNEL INVOLVED. FALL WIOL NOT BE CLEANED UP.

A ROOF FALL HAD OCCURRED AT A SLIP MEASURING APPROX. 30' L X 15' X 6' IN THE #5 ENTRY (20+85), 4 XCUTS INBY FEEDER. MSHA WAS CALLED AND INVESTIGATED AND THEN THE FALL WAS CLEANED UP AND RESUPP ORTED. THIS WAS #5 UNTI, 1ST NEW SUBMAIN.
A CONTRACTOR PARKED HIS TRUCK IN A TURN AROUND AREA APPROX. 75 YARDS FROM STOCKPILE OF UNDERGROUND MINE. HE PLACED AN IMPROVISED STAND UNDERNEATH THE REAR OF THE TRUCK TO LEVER THE TRUCK OFF T HE GROUND AS BED RAISED. UNDERNEATH, HE BEGAN WORKING, ADJUSTING BRAKES. TRUCK EITHER ROLLED OR SLIPPED OF STAND PINCHING HIM BETWEEN THE REAR AXLE & STAND. FOUND SITTING ON GROUND UNDER TRUC
PILLAR WORK IN THE NO.2 ENTRY THE LAST LIFT IN THE RIGHT BLOCK WAS FINISHED AND THE MINER OPR. AND HELPER WAS BACKING THE MINER OUT. A PILLAR FALL OCCURED AND FELL BEYOND THE MINER WHITCH CAUG HT THE HELPER INBY THE CORNER OF THE SOLID PILLAR BEHIND THE MINER.
EE WAS FATALLY ELECTROCRATED AS HE DISASSEMBLED A CONVEYOR BELT STARTING BOX SOURCE OF POWER CAUSING DEATH IN UNKNOWN AT THAT TIME.
AN APPARENT BRAKE MALFUNCTION CAUSED THE VICTIM TO EXIT THE CAB & WAS PULLED UNDER THE REAR WHEELS OR HE PARKED IT & CRAWLED UNDER THE BED TO CHECK A PROBLEM. EITHER SENARIO CAUSED THE VEHICLE TO ROLL OVER HIS BODY CRUSHING HIM TO DEATH. THERE WAS NO APPARENT DAMAGE TO THE TRUCK.
THE INJURED STATED HE WAS ASSISTING A MECHANIC IN TRANSFERRING TOOLS FROM A METAL TOOL BIN CART ONTO A HEAVY DUTY PLASTIC CART. AFTER COMPLETING MOVING OF THE TOOLS, HE WAS MOVING THE METAL CA R, WHEN THE MECHANIC PUSHED THE COVER CLOSED, BEFORE THE INJURED COULD REMOVE HIS HAND, RESULTING IN HIS RIGHT RING FINGER BEING SHEARED OFF AT THE TIP.
INJURED'S LEFT LITTLE FINGER WAS SURGICALLY AMPUTATED FOLLOWING AN ACCIDENT WHERE THE FINGER WAS PULLED AFTER BEING CAUGHT IN A ELEVATOR DOOR STRAP. INJURED HAD REACHED OVER A SAFETY GATE TO G RAB THE STRAP WHILE THE ELEVATOR WAS MOVING.
EMPLOYEE WAS HELPING REPAIR THE R-BOLT MACHINE. HE WAS USING A LARGE WRENCH TO REMOVE A CONNECTION ON THE MACHINE, AND IN ORDER TO DO THAT HE PULLED UP ON THE WRENCH FOR MORE LEVERAGE. WHEN HE DID THAT HE FELT PAIN IN THE GROIN AREA. EMPLOYEE JUST RETURNED TO WORK FROM A SIMILAR INJURY AT ANOTHER JOB SITE. HE RE INJURED AN OLD INJURY.
A 35-TON MOBILE CRANE CONTACTED HIGH-VOLTAGE POWER TRANSMISSION LINES WHILE MOVING AN EMPTY 5,000 GALLON WATER TANK. A MINER TENDING THE TAG-LINE RECEIVED SERIOUS ELECTRICAL BURNS AS A RESULT OF THE CONTACT.
INJURED WAS CHECKING FOR LEAKS ON GENERATOR. HE REMOVED GUARDS FROM THE GENERATOR AND ACCIDENTLY STUCK HAND INTO MOVING FAN.
EMPLOYEE WAS INSTALLING A WALKING BEAM IN A LUBE TRUCK. HE WAS RAISING THE BEAM(ONE END) INTO POSITION WITH THE AXLE HOUSING USING A 3/4TON COMEALONG. EMPLOYEE WAS UNDER THE TRUCK WATCHING THE ALIGNMENT. AS THE BEAM WAS LOWERED INTO POSITION, THE BEAM GOT STUCK.ANOTHER EMPLOYEE TAPPED THE BEAM W/A HAMMER-CAUSING THE BEAM TO FALL TO THE GROUND ON INJUREDS FINGER.SLACK IN COMELONG.
EE WAS ATTEMPTING TO HOOK UP A SCREEN DECK TO BE LIFTED WHEN ANOTHER EE SHIFTED THE DECK CAUSING HIS FINGER TO BE CAUGHT BETWEEN THE DECKS.
WAS STACKING STONE ON A PILE AND STEPPED INTO A JAGGED PIECE OF ROCK, PUNCTURING LEFT LEG. (NO STICHES, IS ON ANTIBIOTICS FOR INFECTION AND IS OFF WORK AT THIS TIME).

EE WAS WORKING NEAR THE FEED CHUTE AREA ON SB PEBBLE MILL. HE TRIPPED ON AN AIR HOSE AND FELL. HE WAS EVALUATED BY A PHYSICIAN AND ON 2/11/02 HAD SURGERY ON HIS ELBOW.
CHANGING CUTTING EDGE ON 1845 CASE SHID SLEER HELD HEAD OF A BOLT WITH LEFT HAND RUNNING AIR IMPACT WRENCH WITH RIGHT HAND BOLT HEAD SPUN CUTTING INDEX AND MIDDLE FINGER ON LEFT HAND. RECIEVED 5 STITCHES IN INDEX FINGER 11 STITCHES IN MIDDLE FINGER.
EE HAD JUST ARRIVE AT THE LONGWALL SHEARER AND WAS PREPARING TO CHANGE SOME OF THE BITS ON THE TAIL AND DRUM. HE HAD HIT A COUPLE OF THE BITS, TO LOOSEN THEM, WHEN MATERIALS FELL FROM THE MINE ROOF, STRIKING HIM ON THE HEAD, NECK, AND RIGHT SHOULDER. THE MATERIALS WERE A COMBINATION OF ROCK AND COAL AND WAS APPROX. 30"" X 36"" X 1 1/2"" THICK.
WHILE DOING ROUTINE MAINTENANCE ON #5 ROLL CRUSHER, HE STEPPED OUT ONTO A BOARD THAT HE HAD PLACED OVER CHUTE. BOARD SLIPPED & HE FELL 3' SUSTAINING BILATERAL SHOULDER STRAINS IN THE ATTEMPT T O BREAK HIS FALL.
EE WAS OPERATING A ROCK TRUCK. AS THE LOADER WASDUMPING MATERIAL INTO THE TRUCK BED A ROCK ROLLED FROM THE BUCKET AND STRUCK THE BED HARD ENOUGHTO TOSS EE IN THE TRUCK CAB. HE SOUGHT MEDICAL A TTENTION AND BEGAN LOSING WORK ON 8/24/01 DUE TOMUSCLE SPRAIN.
THE EE STRAINED HIS BACK WHILE PULLING AN ULTRAVAC HOSE. THEN LATER INT HE WEEK, THE EE JARRED HIS BACK WHILE OPERATING THE PAYLOADER.

TWO EMPLOYEES WERE ATTEMPTING TO CHANGE THE V-BELTS ON THE #2 PRODUCT CONVEYOR DRIVE UNIT. ONE EE DISCONNECTED THE TURNBUCKLE FROM THE DIRVE UNIT. THE UNIT ROTATED FORWARD AND FORCED HIS FACE INTO THE FRAMEWORK OF THE BALLAST WASH SCREEN.
EE WAS INVENTORYING SUPPLIES ON BOLTER WHEN A PIECE OF ROCK 3' X 3' X 8"" FELL FROM ROOF STRIKINGHIS HEAD. HE WAS CHECKED AT HOSPITAL & RELEASED SAME DAY. HE WORKED NEXT 3 DAYS, TOOK VACATION & FIRST DAY OFF WORK WAS AUGUST 6, 2001.

EMPLOYEE REPORTED EXPERIENCING PAIN IN HIS LOWER BACK, HIPS AND LEFT LEG WHILE HE WAS ATTEMPTING TO SUSPEND A CONTINUOUS MINER TRAILING CABLE FROM THE ROOF.
EE WAS HELPING MECHANIC TAKE A SHUTTLE CAR TIR3E OFF OF A TRAILER WHEN THE TIRE FLIPPED OFF OF THE TRAILER AND THE EE TRIED TO HOLD IT AND STRAINED HIS LOWER BACK.
EE COULD NOT START THE SHACKER SO HE WENT TO THE ELECTRICAL BUILDING TO CHECK THE HEATERS AND FUSES. EE HAD BEEN WORKING ON THE PLANT AND WAS HOT AND SWEATY. HE FOUND TWO BLOWN FUSES AND BEGAN REPLACING THEM WITH DISCONNECT STILL ON. WHEN HE TRIED TO INSET THE SECOND FUSE HE RECEIVED AN ELECTRICAL SHOCK.

WHILE ASCENDING LADDER TO GET INTO TRUCK EMPLOYEE FELT A POP IN HIS BACK WHILE AT THE TOP OF THE LADDER. TAKEN TO THE E.R. SAME DAY AND WAS RELEASED TO LIGHT DUTY AND BEGAN TO HAVE DISCOMFORT AGAIN 7-26-01. SOUGHT TREATMENT AND WAS TAKEN OUT OF WORK.
EE WAS CLEANING OFF SIDE BOARDS ON THE DUMP BED OF A GOB TRUCK AFTER DUMPING COAL REFUSE. WHILE CLIMBING DOWN THE LADDER ON THE SIDE OF THE DUMPBED HIS GLOVE GOT CAUGHT. WHEN HE TRIED TO FREE HIS GLOVE HE FELL APPROXIMATELY 4' TO THE GROUNDLANDING ON HIS BACK.
WHILE BACKING AN OVERBURDEN TRUCK DOWN A SLOPE TO DUMP THE LOAD, THE TRUCK OVERTURNED ONTO ITS SIDE, CAUSING A BRUISE TO EE'S LEG AND HIPS. HER FIRST DAY MISSED WAS 7/24/01.

THE INJURED WAS STANDING ALONGSIDE OF AN 18' LONG X 6"" ""H"" BEAM AS IT WAS BEING DRAGGED IN THE TRACK ENTRY BY A SCOOP HOOKED WITH A CHAIN, WHEN THE END OF THE BEAM CAME IN CONTACT WITH A FISH PLATE, CAUSING THE OTHER END OF THE BEAM TO RAISE UP FROM THE MINE FLOOR, SWING SIDEWAYS AND STRIKE THE INJURED ON THE RIGHT SIDE OF HIS HEAD, RESULTING IN A LACERATION & KNOCKED HIM UNCONSCIO
EE STATED THAT HE WAS WALKING BESIDE A MAINTAINCE VEHICLE WHEN HIS RIGHT FOOT SLIPPED ON AN AREA OF SLICK BOTTOM. HE STATED THAT BEFORE HR EGAINED HIS BALANCE HIS RIGHT KNEE HYPEREXTENDED CAUS ING PAIN IN AND AROUND THE RIGHT KNEE. EE MISSEDFIRST SCHEDULED SHIFT ON 8-6-01.

THOUGHT ARM WOULD BE OK, THE N ON 7-21 NOTICED SWELLING AND BRUISING DX-LT ARM BICEP TEAR. TURNED IN LATE DUE TO WAITING ON MEDICAL RPTS AND INFO TO CONFIRM INJURY.

EE WAS SHOVELING ROCK FROM UNDER ROCK CRUSHER AND PULLED A GROIN MUSCLE. EE DID NOT SEEK MEDICAL ATTENTION UNTIL 7-23-01. EE WAS DX WITH A RIGHT INHGUINAL HERNIA SECOND DEGREE SHOVLING. EE WAS OUT FORE 42 DAY AFTER A 2"" HERNIA REPAIR.

HE INJ HIS LOWER BACK & HAD A SHOOTING PAIN IN HIS LEFT LEG. THE HOSP HAS SCHEDULED HIM FOR AN MRI HE WILL NOT RETURN TO WORK UNTIL WE GET RESULTS OF MRI & HAS FULL DRS RELEASE.
EMPLOYEE WAS PULLING VENTILATION CURTAIN OUT FROM UNDER LOOSE COAL THAT HAD FALLEN ON IT AND HURT HIS BACK.
EE WAS OPERATING A MINER IN #6 ENTRY #3 UNIT, STANDING BETWEEN PINS. A ROCK FELL OUT AND LANDED ON HIS RIGHT FOOT. THE ROCK WAS APPROXIMATELY 62"" X 42"" X 0-6"" THICK.
EE WAS USING LEFT LEG TO FREE AN ELECTRICAL CABLE BEING PULLED WITH A WINCH THAT WAS HUNG IN A PULLING SHIVE. CAUSED HERNIA.
THE EMPLOYEE WAS UNLOADING 5 GAL CANS OF WHITE OIL FROM THE SCOOP WHEN A HANDLE BROKE OFF A CAN.CAUSING HIM TO TWIST HIS BACK AND RIGHT KNEE. THE EMPLOYEE SOUGHT MEDICAL TREATMENT AND SURGERY WAS SCHEDULED FOR NOV 29, 2001 AND NOV 28 WAS HIS LAST DAY AT WORK.

EE STARTED HIS SHIFT ON AN L85 ROCK TRUCK AT 0600. LATER IN THE SHIFT HE STATED THAT HIS SEAT WAS LOOSE CAUSING HIS BACK TO HURT. A NEW SEAT WAS ORDERED. AT 1000HE PARKED THE ROCK TRUCK, STATE D HIS BACK WAS HURTING AND WENT HOME. HE HAD NOT RETURNED TO WORK.

INJ EE WAS USING A CRESCENT WRENCH ATTEMPTING TO HOLD A COUPLER IN PLACE AS OTHER EE USED A PIPE WRENCH TO TIGHTEN THE BOLTS ON THE SAME COUPLER. WHILE PERFORMING THIS TASK INJ EE EXPERIENCED SOME PAIN IN UPPER RIGHT ARM HE IS NOW MISSING WORK DUE TO PROBLEM HOWEVER COMPLETED DIAGNOSIS HAS NOT BEEN MADE

WALKING IN 3RD NORTH PANEL #6 ENTRY BETWEEN 3 AND 4 CUT, STEPPED IN HOLE AND TWISTED HIS RIGHT KNEE.
EE WAS GOING DOWN A SMALL SAND RAMP TO RELOAD. LOST CONTROL OF HIS TRUCK WHICH CAUSED HIS TRUCKTO BOUNCE HARD AND HIGH. THE HARD BOUNCE CAUSED HIM TO STRAIN HIS BACK, NECK AND SHOULDER.

WHILE GOING TO THE OIL SUPPLY (WALKING ON THE GROUND), EE LOST HIS FOOTING (SLIPPED) AND FEL L ONTO THE GROUND INJURING HIS LEFT THIGH, RIGHT SHIN, RIGHT SHOULDER AND RIGHT ARM. SITE CONDITIONS WERE WARM, WET GROUND CONDITIONS.
EE STATED THAT HE WAS CROSSING THROUGH THE MANDOOR SEPERATING THE TRACK FROM THE RETURN ENTRY ON 7-RIGHT SECTION AS HE DID THE PRESSURE FORCED HIM TO LOOSE HIS BALANCE CAUSING HIM TO FALL AND TWIST HIS LEFT ANKLE. MOUTH OF SECTION.
EE WAS UNLOADING TIMBERS FROM A SCOOP. WHEN HE APPARENTLY STRAINED HIS GROIN AREA.
EMPLOYEE STATED THAT WHILE MOVING A PIECE OF ROCK, HE FELT A PAIN IN HIS NECK AND LEFT SHOULDER.
EE LEFT BLANK ON ACCIDENT REPORT. STATED ON PHONE THAT NO PARTICULAR INCIDENT, BUT OCCURRED OVER TIME BECAUSE OF REPEATED ACTIVITY OF SHOVELING.
EE ALLEGES HE WAS USING A PRYBAR TO CLEAN OFF BUILT UP MATERIAL FROM THE PRIMARY CONVEYOR AND THE PRYBAR SLIPPED SMASHING HIS LEFT MIDDLE FINGER.
AREA WET AND MUDDY. HANGING CURTAIN AND TWISTED HIS RIGHT KNEE.
SOMETHING FELL INTO SOME DUST AROUND EMPLOYEE SENDING DUST INTO THE AIR. STEEL SUDT GOT INTO EMPLOYEE'S EYE. HE WAS NOT WEARING SAFETY GLASSES. MISSED ONLY 1 DAY AS SUGGESTED BY DOCTOR.
EE SLIPPED AND TWISTED HIS RIGHT KNEE WHILE PERFORMING MAINTENANCE WORK ON A PIECE OF MACHINERY. THE INJURY WAS DIAGNOSED AS MUSCLE STRAIN AND HE CONTINUED TO WORK WITH SOME DISCOMFORT UNTIL 1 2-14-01 AT WHICH TIME HE HAD SURGERY.
EE STATED THAT HE WAS HELPING UNLOAD A BELT CROSSWALK FROM AN OPOSSUM BELLY CAR. AS HE DID HE SLIPPED ON SOME LOOSE MATERIAL THAT WAS ON THE CAR. WHEN HE SLIPPED THE WEIGHT OF THE CROSSWALK CA USED HIM TO TWIST HIS UPPER BODY HURTING HIS BACK.
EE WAS CLOSING ROLL OUT CHUTE USING A 4' BAR ANDTWISTED HIS BACK.
THE EMPLOYEE WAS PACKING THE CEMENT PRODUCTS IN A BUG BAG, ANOTHER EE WAS MOVING THE PALLET WITH THE FORKLIFT. NO ONE REALIZED THAT HIS FOOT WAS UNDER THE FORKLIFT AND HE LOWERED THE FORKLIFT, IT PINCHED HIS RIGHT THUMB.
EMPLOYEE WAS LIFTING A FULL 5 GALLON WATER BOTTLE WHEN HER WRIST POPPED AND STARTED HURTING. AFTER SEVERAL DAYS, SHE STATED THERE WAS STILL MUCHDISCOMFORT, AND SHE WAS SEEN BY DR. WHO TREATED HER FOR A SPRAINED WRIST. THE EMPLOYEE STATED HER WRIST GOT BETTER, BUT WENT BACK TO HURTING AT NIGHT. THE DR. RECOMMENDED A SPECIALIST.SPECIALIST DIAGNOSED SPRAINED WRIST AND WRIST BRACE.
EMPLOYEE WAS TAKING DOWN CURTAIN WHEN HE TWISTED LOWER PART OF BACK.
THE INJURED PUT SOME USED ANVILS ONTO A PALLET WHEN HE NOTICIED PAIN IN HIS BACK. THE ANVILS WEIGHED ABOUT 80 POUNDS.
EE WAS LIFTING A AG OF HAND TOOLS OUT OF A TOOL BOX AT THE BELTMAN'S SHANTY. EE STATED HE FELT SOMETHING PULL IN HIS LEFT SHOULDER. NOTE: CONTINUED TO WORK UNTIL 7-23-01-FIRST DAY OFF WORK.
EE ALLEGES THAT WHILE HE WAS WALKING DOWN STAIRS, HE SLIPPED ON A BALL IF CLINKER TWISTING HIS ANKLE. LEFT ANKLE IS FRACTURED.
TWO EES ALLEGED THAT THEY WERE REPAIRING AIRLINES OPN SCRAPER WHEN ONE MECHANIC WENT TO REPOSITION EJECTOR (MOVE BACK TO REAR POSITION). INJURED EE WENT BACK IN BETWEEN EJECTOR AND REAR FRAME OF SCRAPER. RIGHT LEG WAS CAUGHT IN PINCH POINT RESULTING IN BROKEN FIBULA.
AT END OF SHIFT, HAD JUST FINISHED BOLTING FACE, EE WAS WALKING AROUND BOLTER WHEN A ROCK FELL FROM RIB TO BOLTS STRIKING HIM ON LEFT LEG AND KNEE.
EE WAS WORKING ON A CREW ADVANCING C/M SECTION POWER CENTER AND CABLES. AS HE ATTEMPTED TO TAKEDOWN A CABLE FROM A HOOK A SMALL PIECE OF SLATE DISLODGED, FELL & STRUCK THE LEFT SIDE OF FACE.
EE WAS WAITING TO BE LOADED WHEN THE LOADER OPERATOR DROPPED THE BUCKET INTO THE BED OF THE TRUCK. THE JAR CAUSED HIS AIR SEAT TO BOTTOM OUT INJURING HIS BACK.

EE WAS ADDING A SECTION OF DRILL STEAL TO REACH THE DEPTH OF THE HOLE, WHEN HE BACKED THE FEED OFF FROM FAST FEED THE DRILL BOUNCED HITTING EE'SLEFT SIDE. EE HAS HAD 32 HOURS OF INSTRUCTIONAL TRAINING AND WAS BEING SUPERVISED AT THE TIME OFINJURY. EE WAS CARELESS, NOT PAYING ATTENTION.

WHILE THE EE WAS ASSISTING WITH MAKING A BELT MOVE INSTALLING BELT STRUCTURE HE STRAINED HIS LOWER BACK.

OPERATOR CLIMBED OFF LOADER TO WASH BUCKET OUT AND SLIPPED ON LOOSE GRAVEL AND TWISTED LEFT ANKLE.
OPERATOR SLIPPED OFF STEP OF DOZER WHEN EXITING TWISTING LEG (RIGHT).
EE WAS OPERATING A JITNEY RAILRUNNER WHEN HE FAILED TO SLOW DOWN AT THE OLD 4 SPUR SWITCH WHICH WAS BEING USED A DERAIL. HE RECKED AND CAUSED IMPACK TO HIS TAILBONE. HE COMPLETED HIS SHIFT AND WENT TO DR ON THE 18TH.
THE INJURED EE WAS ROTATING THE 2J LW TG DRUM BY HAND WHEN HE WAS STRUCK ON THE LT. HAND BY A PIECE OF FALLING ROCK. SUFFERED FRACTURE TO LT. THUMB & INDEX FINGER.
WAS GETTING OFF FUEL TRUCK WITH FUEL HOSE. STEPPED ON ROCK TWISTED RIGHT ANKLE. TRIED TO CATCH SELF WITH LEFT FOOT TWISTED IT. NOTE: EE DID NOT START MISSING WORK DUE TO THIS INJURY UNTIL 7/24 /01.
