Roof fall found in 1st right in the #3 entry x-c17 across from the #3 belt in belt neutral. Fall was 60' x 60' x 6-10' thick. The belt access point had already been cribbed and timbered before fall and after fall all other access points were cribbed off. Area mined and bolted around Dec.2001 with 6' full grout resin bolts thru a slip in top. No plans to reclaim.

NORTH 2, 69 BLOCK, 6 ENTRY, 4-WAY INTERSECTION, MMU 004. 20'WIDE X 20'LONG X 8'HIGH. SUPPORT: 6'3/4"" RESIN BOLTS, PANS, TWELVE 12' CABLES BOLTS. FALL DANGERED OFF, ADDITIONAL ROOF SUPPORT INSTALLED AROUND FALL.
A METHANE IGNITION OCCURRED IN THE PRODUCTION SHAFT DUE TO A SPARK OR SLAG FROM A CUTTING TORCH. THE WORK WITH THE CUTTING TORCH WAS ON THE UPPER DECK OF THE PROD SHAFT HEAD FRAME, APPROX 650' ABOVE WHERE THE FLAME WAS DISCOVERED IN THE SHAFT. THE FLAME WAS APPROX 6"" LONG COMING OUT OF THE SHAFT WALL & WAS EXTINGUISHED BY A FIRE EXTINGUISHER. NO INJURIES OR EQUIP DAMAGE.
At around 4:30 p.m. the elevator at our Hart street portal failed to operate. It was found that two switches at the bottom of the elevator shaft malfunctioned. The malfunctioning switches were the buffer and overtravel ones. The switches were replaced and the elevator was returned to service at around 6:30 p.m..
An unintentional roof fall occurred in the South East Main return, entry #6 x-cut 22+40. The cause of the roof fall was laminated and unconsolidated shale above the ancharage point. The area had 6' fully grouted rebar roof bolts and wire matting installed for roof support.
THE FALL WAS IN THE #6 NORTH ENTRY (SUPPLY ROAD) AT +5321 TO +5241 IN PILLAR LINE. WAS DEVELOPED APPROX 8/2/04. THE ROOF WAS PINNED WITH 5' TENSION REBAR BOLTS. THE FALL WAS APPROX 50' LONG, 18' WIDE, 5' HIGH.
Unintentional roof fall ocurred in Return Air Course. 7000' outby 005 on 2nd Northwest mains in #2 entry, spad #476. 30' x 19' & approx. 6'thick. Appeared to be weathered & shale roof. Fall will be cleaned up & bolted back with extra support added.
An unplanned face ignition occurred in the #1 right break on 002 section.
Fall in #4 entry (spad #37468 2 x-cuts inby) in #2 unit intake. Length 60 ft, width 18 ft, thickness 12 ft.
Spontaneous Combustion in a coal stockpile.
The Lakeshore elevator at the Williamsville portal stopped in mid shaft with 15 employees aboard. Elevator was down for approx. 40 min. until it could be manually lowered. At which time a door proximity roller was replaced and elevator was put back into service.(11:47 pm to 12:27 am. Maintenance personal had to use key to open the door. repair damage linkage.)
There was a ground fall at the Deep Post mine, Deep Star side of the mine on the 4570 level. This ground fall impeded passage to the work area. MSHA mine inspector was notified and went to the area.

FALL OF ROOF, 003-0 SECTION 2 RIGHT, 10 SOUTH MAIN WEST ENTRY AT SS 6 + 95. APPROX 40' LONG X 25' WIDE X 6' - 7' HIGH. GREY SHALE
A friction caused methane ignition occurred at approx. 8:43 pm on the panel 51 MMU 011-0 longwall face. A Horizontal degas hole was encountered between 173 and 174 shield and it was not visible due to the softness of the coal. The head drum was running at roof level and just started to contact the face when the head drum operator saw the ignition. the flame were 18"" high
Production hoist outage. A brake valve on the production hoist would not release at the beginning of afternoon shift. The mine was evacuated to repair the problem at approximately 3:20 p.m. MSHA field office notified by phone. A brake valve and brake release micro-switch were replaced. The repairs were tested and employees returned into the mine at approximately 5:45 p.m.

HAD AN UNINTENTIONAL ROOF FALL IN THE #3 ENTRY 5 BREAKS OUTBY THE FACE OF 011 MMU.
Employee arrived at work around 12:00 am; he noticed on the surveillance camera that the crude stockpile showed gray color on the screen. He thought it had fallen down; he then took off the dryer area and saw a flame coming from a pile of coal under the coal bin tank. At that time two employees came up and together they put out the fire with water. It took about1:20 minutes.
A roof fall was discovered on the preshift examination in the belt entry 900' inby the surface. The fall measure 30' in length 17' wide and 8' thick. the fall occurred over holiday weekend. and no one was in the mine.
A roof fall occurred on #2 Sect - #1 Mains. 2 breaks inby spad 4099 in the #3 Entry. The fall measured approximately 18 ft wide, 40 ft long and 10-12 ft high.
Unintentional roof fall - 6 North - No. 2 entry at #32 4-way intersection. Area is in intake escapeway for 5-west section.
An explosion occurred underground in the area of crosscut 24-25, entry #1, near the underground compressor. The explosion was believed to have been caused by a low voltage signal coming from the power company causing the main mine fan to go off, but not the power to the underground areas of the mine. No one was on the mine property when the explosion occurred.
Garden A-Shaft automatic elevator was inoperable due to the tread wear switch out of adjustment.
Impoundment failure over four day holiday weekend (Thanksgiving).
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON THE NORTH EAST MAINS, #4 ENTRY (RETURN) @ 39 BLOCK, SPAD STATION #3781. THE FALL MEASURED 18' HIGH, 20' WIDE, 70' LONG. TORQUE TENSION 5' RODS WERE USED IN THIS AREA. FALL WAS INVESTIGATED BY M.E. (MSHA)
An unintention fall occurred on 20 travelway at the 122 x/c 78+35' fall was 20' long by 20' wide by 10' high-travelway was reouted and didn't affect air.
Existing Roof Fall from 1997 fell out again was found on the intake travelway about 10'long x 20' wide x 10' thick by the 3rd shift fire boss at 9pm as he was going in to do preshift exam. Fall was in the #4entry around 320' inside portal. The mines had been idled since 11-23-05.
An unintentional roof fall was discovered one break inby spad #2017. The fall measured approximately 20' wide by 20' long by 4'-5' high.
Location of fall: No.1 section - 9 belt; entry: 4; spad no. 9412 one break in-by. Distance from active face: 810 ft. Airway: neutral. Size of fall: length: 60ft. width: 19ft. height: 4ft. Roof fall on belt line at crosscut #56. Fall was 14'high x 16'wide x 60'long. Roof was bolted with 5' resin bolts. Area mined Oct. 2005.
An unintentional fall occurred in the 3rd east at #51 x cut 35+70'. Fall was 20' long 20' wide and 9' high. Ventilation and passage of min was not affected.
A ROOF FALL HAS OCCURRED IN THE MNO #2 #10 ENTRY AT SPAD 10+54. THE FALL OCCURRED ABOVE THE ANCHORAGE ZONE. THE ENTRY IS NOT A NORMALLY TRAVELED ENTRY. THE FALL MEASURED APPROXIMATELY 5 FEET HIGH AND 25 FEET LONG AND HAS BEEN DANGERED OFF. THERE WAS NO DISRUPTION OF WORKFORCE, BLOCKAGE OF TRAVELWAYS OR AIR AS A RESULT OF THIS FALL OR DAMAGE TO ANY EQUIPMENT.
Garden A-Shaft automatic elevator was inoperable due to the E-Stop button stuck in on the bottom landing.
The Mary Ann hoist went down due to a ground fault from a pinched wire on a limit switch cover. It went back up at 6:10 pm on 11/27/05.
A roof fall occurred in the intersection of the #2 heading at break #23. The fall extended over into the adjacent crosscuts. The fall was 40 feet long and 19 feet wide.
Roof fall at spad 9+35 to 9+65 in #3 entry north j-2 panel. fall was 10' high, 18' wide, 30' long. No injury and No equipment was involved int he fall- the area has been cribbed and dangered off.
The Tyro Creek service elevator was out of service due to the governor rope having too much slack in it causing the limit switch to come on. Bagby Elevator Company took out 16 inches of rope and the elevator was operational again at 5:10 p.m.
During a normal electrical P.M. on the hoist, the hoist would not fire back up. Electricians traced the problem to PLC communication failure. Replaced the CRP card. Hoist fired back up and all hoist checks o.k.

HAD UNINTENTIONAL ROOF FALL ON #15 BELT AT BREAK NO. 75
A fall occurred in the #3 entry, #7 break on the 8 West section. The fall length was 35 feet long and 18 feet wide.
A fall of roof occured in the #3 entry at the #3 crosscut intersection on West Mains Section. The fall was 15 ft wide, 15 ft long and 8 1/2 ft high. Area was supported to plow by 6 ft double-lock bolts and resin rib bolt. no equipment involved, no injuries.
A rock fall was discovered int he #5 entry of the Bii Panela: in the #49 crosscut. Fall was approx. 30' x 20' x 8' long. Fall was 4 breaks outby the face of the 005-0 section. The area orignially was bolted with 5' hi-tech bolts; and some 8' super bolts added for supplemental support. MSHA was notified of the fall.
a rock fall was found during an exam of the left return entry, the fall was 4 breaks or about 250 feet underdround. It fell in the intersection and was approximately 4 feet thick, and 20 feet long, 20 feet wide. no one was injured by this incident.
11/23/05 @ 12:55 pm-Gas was reported outby in the LOC @ the B1 Seals-18.1% O2, 3.7% CH4. 01:26 pm-MSHA-K. Roberts was informed. 02:00 pm-spoke with MSHA-K. Roberts, D. Whitcomb, & J. Oakes. 02:10 pm-called MSHA, High CH4 @ ME Seals. All power off in mine. 2:35 pm-the ME Seals showed-20% O2, 2.4 CH4. 02:45 pm-all miners were out of the mine, informed MSHA-D. Whitcomb.
A rock fall occurred at #6 entry #1 unit fro 8+50 to 10+00. The unit has moved out of the panel. MSHA district office was notified.
An unintentional rock fall occurred on #1 unit #3 entry inby 3+50 to inby 4+50. This is a neutral entry - the face is at 11+00. The fall will not be cleaned up at this time.
Roof fall 9,000 feet outby section in upper seam, old works on tunnel belt, neutral entry No. 2 at Spad 1106, 1st. East Mains. Will not be rehabilitated.
A roof fall occured in the 1 South, crosscut #38, starting in the pillar line between #6 and #7 entry proceeding through the intersection of #7 entry. The roof fall measured approximately 35'X 19'X 6'. An undetected slip was determined to be the cause of the fall. The area was supported with 6' tension rebar bolts and the intersection of #7 entry was xd-out with 9' mega bolts.
A ROOF FALL WAS OBSERVED IN THE MAIN NORTH ENTRY (#9) STARTING AT SPAD #51+80 INBY TO SPAD #56+30. APPROX 45O LONG. FALL IS INBY 4E PANEL SEALS.
AT 1:40 PM, A ROOF FALL (20' W X 300' L X 8' H) OCCURRED IN #6 ENTRY 1 BRK FROM THE FACE. BOTH MSHA & OMSL INVESTIGATED THE FALL, WHICH WAS BREAKERED OFF (BLOCKED) AND WE ARE GOING AROUND.
Security guard discovered fire. No employees were at the site. Fire of unknown origin destroyed the control room. Control room was constructed of wood and tin. Control room was the point of electrical feed into plant.
ON 11/22/05 AT 5:45 PM, THE NORTH SKIP ON THE 16' SHAFT TRAVELED OPUTSIDE OF THE SURFACE SPEAR GUIDES DAMAGING THE CANOPY ON THE SKIP. ALL EQUIPMENT WAS INSPECTED FOR SIGNS OF DAMAGE OR WEAR WHICH COULD HAVE CONTRIBUTED TO THE INCIDENT. NO ADDITIONAL DAMAGE WAS IDENTIFIED. REPAIRS WERE MADE AND THE SKIP WAS TESTED THEN PLACED INTO SERVICE.
SCR of main drive motor is bad. Replacing SCR & starter on motor. Parts on site. Approx time of return to service 11/28/2005. Firing pulse generators bad & need to be rebuilt. Man hoist & emergency hoist will be manned & ready until elevator is returned to service. Spoke with the Lexington KY MSHA office reporting elevator outage on 11/21/2005 @ 7:00am.

A methane ignition occurred while cutting on the longwall crusher during set up. spaker inginited a chg bleeder along the roof. The flame was short duration and put out with less than a fire extinguter.
Fall occurred on intake, #4 entry over previously installed arches. Fall was 12'W x 20'L x 2' thick. Area will be cleaned and the arches reinstalled.
HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 11/21/2005 at approximately 12:40 pm, the Cowling Portal Elevator experienced a mechanical component that went bad. The coupler between the motor and reducer. The reducer side was stripped out. The elevator was back up and the mechanical component was functioning properly at approximately 07:00 pm.
HAD FALL IN INTERSECTION, BELT ENTRY, 2 X-CUTS INBY FEEDER ON #2 UNIT OF 650'WEST PANEL. FALL MEASURED APPROX. 49' X 42' X 6'HIGH. PULLED 4' DOUBLE LOCK ROOF BOLTS. FALL IS BEING CLEANED UP.
An unintentional roof fall occurred at the Galatia North Mine at the #2 Entry 13 West Longwall @ Crosscut 23. The fall was approximately 20' long, 20' wide and 12' thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up.
A roof fall occurred inby the feeder location between #6 west and #7 west at 4+20. The area was initially bolted with 5' install III bolts. Secondary support was added by installing 12' install II long bolts. The immediate top broke up and fell 5' high. The long bolts are still hanging in place. The unit was idle at the time of the fall.
Roof fall @ 11+20 in Southwest Main developed 10/15/03. Fell 50'long, 19' wide and 19' high and had 5' grouted bolts. Fall fell inside of pillar knocking out mandoor and wall affecting ventilation.
A roof fall occured int he dump point cross-cut at 3&60 #2 west. The dump point being at 3 & 60 #5 west. the area was initially supported with 5' install iii bolts. Secondary support was added by installing 12' install ii long bolts. The roof fall exposed slick and sided rock at the anchor point of the long bolts. The roof fall topped out at this point.
A pall was discovered in intake airway 30'l, 20' w, 5-6' thick. Laminated shale with slight water present. Good air flow across fall and a 6' walkway was timbered across fall edges of fall were sloped with scoop to make travelable. Additional supports were installed in roadway and crosscuts between intake & roadway and between roadway and belt.
A roof fall occurred on the longwall belt approx. 100' long, 20' wide, and 12' high. This area was supported with 6' torque tension bolts as primary and supplemented with cable trusses. This fall was cleaned up and resupported.

THE FALL MEASURED APPROXIMATELY 45' LONG X 20' WIDE X 5'-6' THICK AND WAS COMPOSED OF LAMINATED SANDY SHALE. THE FALL CAUGHT THE 001-0 SECTION BELT LINE IN THE AREA OF THE FALL.
A ROOF FALL MEASURING 40'LONG X 18'WIDE X 8'HIGH OCCURRED IN #0 ENTRY, 1300' OUTBY 003 SECTION AT SPAD NO.6570. AREA WAS BOLTED WITH 5'BOLTS.

An unintentional roof fall occured on the C-Mains section, in #7 entry and cross cut 7 to 8. The roof is adverse and as the ""A"" run in the crosscut was holed into #8 entry the roof began to work. Equipment was backed out and it fell shortly afterward.

A roo ffall occurred in the no. 4 belt entry of no. 1 north (2 breaks inby survey station no. 163. the fall measured approx. 45' long by 20' to 35' wide x 7' high. the area was mined in 1948 and is approx. 4 miles outby the altrp working section. the fall will be clean up. there were no injuries.
An unintentional fall occurred on old 3 c Travelway at #29 xcut-fall was 19'w x 19'l x 8'h. area was bolted with 48"" bolt preveisouly used in this area.
While doing weekly exam located a roof fall at 92 bld old 1 rt. panel #4 bldg. which is common with 3 other bldg. 1 rt is not a active section. The entry at the headgate of the longwall caved in during the production shift. The top at the cave site was loose laminated shales with coal spars. Those contributed to the fall. The cave was measured at 8 ft high x 17'-18' wide x 15' to 20' long. The fall has been cleaned up.

HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 11/14/2005 at approximately 07:00 pm, the Cowling Portal Elevator experienced an electrical component part that went bad. The field power supply had to be replaced. The elevator was back up and the electrical component was functioning correctly at approximately 08:50 pm
ROOF FALL : (7000-1-014-05) LOCATION - Main E Roadway, Crosscut #104, Entry #4. DIMENSIONS - H:6-8', W:25', L:25'. GEOLOGY - soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' fully grouted bolts.
An unintentional roof fall occurred at the Millennium Portal Mine at the 1 North Headgate No. 2 entry at 5617 north. The fall was approx. 90' long, 18' wide and 12' -16' thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up.
An unplanned roof fall occurred in 1 West Room in x-cut #3 to #2, 3 x-cuts inby Survey Station #156. Fall was approximately 4' high, 18' wide, 40' long. Area was bolted with 42"" fully grouted roof bolts according to the approved roof control plan. Area timbered and dangered off.
A roof fall occured in the 1 East Panel, #7 entry in the pillar line between crosscut #12 and crosscut #13 and part of the intersection of crosscut #13. The fall measured approximately 30'X 19'X 6'. The area was supported with 6' tension rebar bolts and 9' mega bolts.
A FALL OCCURRED IN THE #5 ENTRY AT SPAD 215 MEASURING 17' LONG, 19' WIDE, 4.5 HIGH. THE FALL AREA HAS BEEN TIMBERED OFF AND DANGERED OFF. THE FALL WILL NOT BE CLEANED UP. THIS FALL OCCURRED APPROX 1800' FROM FACE.
A ROOF FALL WAS DISCOVERED IN THE NO. 4 BELT ENTRY OF NO. 1 NORTH (1 BK INBY SURVEY STATION NO. 163). THE FALL MEASURED APPROXIMATELY 40' LONG X 20' WIDE X 8' HIGH. THIS AREA WAS MINED IN 1998 AND IS APPROXIMATELY 4 MILES OUTBY THE ACTIVE FACE. THE FALL CAUGHT THE BELT CONVEYOR FOR THE MINE AND WILL BE CLEANED UP. THERE WERE NO INJURIES INVOLVED.
An unplanned roof fall occurred in the #6 entry. The fall started at spad #16586 and extended 90' inby that point. The fall measured 90' length, 18' width and 6' to 7' high. No injuries were reported. All entrances have been cribbed off.

A ROOF FALL OCCURED INBY THE LOADING POINT OF THE #6W INTAKE ENTRY AT THE INTERSECTION LABELED 3+00. THE ROOF CONDITIONS IN THE AREA HAD BEEN CONTINUALLY MONITORED SINCE THE AREA HAD BEEN MINED. ADDITIONAL ROOF SUPPORT HAD ALREADY BEEN INSTALLED. THE CAUSE OF THE FALL WAS DUE TO WATER IN THE UPPER ROOF. INITIALLY BOLTED WITH 5' INSTALL III BOLTS. ADDED 12' INSTALL II BOLTS.
A ROOF FALL OCCURED INBY THE LOADING POINT OF THE #5W BELT ENTRY AT THE INTERSECTION LABELED 3+00. THE ROOF CONDITION IN THE AREA HAD BEEN CONTINUALLY MONITORED SINCE THE AREA HAD BEEN MINED. ADDITIONAL ROOF SUPPORT HAD ALREADY BEEN INSTALLED. THE CAUSE OF THE FALL WAS DIRECTLY RELATED TO WATER IN THE UPPER ROOF. THE AREA WAS BOLTED WITH 5'INSTALL II BOLTS.

Brake car locked 100 feet up from ""Y."" Repairs were made and brake car put back in service at 3:55 p.m.
An unintentional roof fall was discovered on 3 East Mains, #4 Entry, Brk. 12, 150 feet inby spad #18990. The fall measured approximately 20 ft. wide, 60 ft. long, and 6 ft. high. This fall will not be cleaned up. No equipment was involved, ventilation was not impaired and there were no injuries.
A HYDRAULIC FIRE WAS DETECTED ON THE D-11 DOZER ON JOB 234 AT 1:30 AM. THE FIRE WAS FIRST DETECTED ON THE LEFT SIDE OF THE MACHINE UNDER THE CAB AREA. CREW USED ALL POSSIBLE MEANS TO PUT FIRE OUT WITH NO SUCESS. THE BERLIN VFD EXTINGUISHED THE FIRE WITH CHEMICAL FOAM. OPERATOR HAS 25 YEARS EXPERIENCE - 12 YEARS ON D-11.
The # 1 Hoist was down due to a power failure at the Tri-State # 69 substation caused by a bad jumper. The power failure effected all of the Molycorp facility. All of the underground personnel were evacuated from the underground mine. Power was re-established at 9:11 AM. The hoist systems were checked and found OK.
A ROOF FALL OCCURRED IN THE MAIN SOUTH #5 PANEL, #7 ENTRY, CROSSCUT 52 TO 53. THE FALL MEASURES APPROXIMATELY 60 FEET LONG, 20 FEET WIDE, AND 5 FEET HIGH. THE AREA IS SUPPORTED WITH 4 FOOT FULLY GROUTED ROOF BOLTS.
Dry Lenses that were in the engine compartment were ignited by a from spark thus burning fuid line into which burnt the rust of the machine.
The Cedar Creek elevator became out of service due to a malfunction in the E and EL circuit. Bagby Elevator was notified of the problem and dispatched two service techinicians to correct the problem. The elevator was placed back in service at 3:00 a.m. 11/7/05.
On Sunday, 11/6/05 there was a power bump at the Meikle Mine which knocked all ventilation offline. All fans tripped at 4:45 pm. The bulk air cooler fans were online at 4:45 pm, the two main exhaust fans came back online at 5:24 pm. All personnel were evacuated from both Meikle & Rodeo mines until ventilation was returned to normal.
Area had been mined 26 years prior. Area was 190' inby #3 portal. Hill seams were present and had additional support. Fall occurred just inby additional supports. Fall measured 35' x 20' x 5'. Existing support was 48"" resin. Area has been rehabed.
During a routine inspection of the airshaft, mine supervision attempted to activate the escape hoist. Upon activating the switch, the conveyance failed to move. Supervision contacted surface electricians and evacutated the mine.
ROOF FALL (7000-1-012-05) LOCATION - Top of Fault Hill, East of Spad #166. DIMENSIONS - H:6-6.5', W:19', L:120'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' conventional bolts. The area was mined in March 1974. Prominent roof jointing or slips in the roof were the apparent cause for the fall.
AN EE WAS ATTEMPTING TO DISCONNECT A FAIRCHILD 35-C SCOOP FROM THE CHARGING SYSTEM. WHILE DISCONNECTING THE CHARGING PLUG A SPARK OCCURRED FROM A GROUNDED LEAD CAUSING AN IGNITION OF HYDROGEN GAS THE HAD ACCUMULATED UNDERNEATH THE LIDS OF THE BATTERY. NO INJURIES WERE REPORTED IN CONNECTION TO THIS IGNITION.

An unintentional roof fall occurred #3 North Belt and Track entry at spad #1902. Size of fall was estimated to be 150'L x 20-22'W x 4-7'H. The area had been supported with 5' fully grouted resin bolts with supplemental support consisting of 9' cable bolts. No injuries, ventilation was not interrupted, belt and track were caught under fall. Fall has been cleaned up.

AT 7 AM THE LOWER PART OF THE DEEP WELL PUMP #4 SHROUD WAS BEING LOWERED INTO THE HOLE. THE OTHER END WAS LOWERED TO CONNECT THE 2 PARTS TOGETHER. AT 705, A METHANE TEST WAS MADE & 0.0 WAS ALL THAT THE SPOTTER READ. AT THAT TIME THE PREP PLANT MECHANIC STARTED WELDING. THAT IS WHEN HE FELT HEAT ON HIS HAND. HE STOPPED, LOOKED AROUND & STARTED AGAIN. IGNITION OCCURRED.

Hoist operator received a call from cager stating he heard something fall down shaft but wasn't sure what it was. Cager called shifter and he checked sump and found 2 pieces of a broken guide. Shaft crew found 2 split guides. Down 3:34 p.m. - Started 1:00 a.m.
THE FALL WAS DISCOVERED BY GENERAL MINE FOREMAN DURING A WEEKLY EXAM. THIS AREA HAS OTHER FALLS NEARBY. THIS PARTICULAR FALL MEASURED APPROXIMATELY 20' WIDE X 40' LONG X 4'-5' HIGH.
Hoist quit operating at 10:10am. MSHA was contacted at 10:40am. A broken relay was identified and repaired and hoist put back in service at 12:40pm.
A rock fall was discovered int he #5 entry of the % west Panel; at the #24 crosscut. Fall was approx. 18' wide x 18' long. Height was undetermined. The area had originally been bolted with 5' hi-tech bolts; and some 8' super bolts added for supplemental support. MSHA was notified of the fall.

THE EE WAS ATTEMPTING TO IDENTIFY LEAKS IN A PONTOON ON A PUMP BARGE. APPARENTLY HE CONNECTED A COMPRESSOR TO THE END PLATE OF THE PONTOON VIA A THREADED NIPPLE, A HOSE WITH A VALVE AND AN AIR PRESSURE GAUGE. THE END OF THE PONTOON RUPTURED AND APPEARS TO HAVE HIT THE EMPLOYEE IN THE HEAD. BECAUSE THERE WERE NO WITNESSES, THE EXACT PROCEDURE & POSITION OF EE'S BODY DURING THE
Disassembling a crushing plant. two employees were attempting to remove the motor that drove the hazmac crusher by using a boom truck. The motor became stuck on the motor mount bolts that had been cut off using a torch. The employees then tried to pry it from its stuck position when it freed and came forward.
AT APPROX 12:10 A.M. NOV. 8, 2005 THE OPERATOR OF A MACK TK TANDEM HAUL TRUCK WAS FATALLY INJURED WHEN THE TRUCK HE WAS OPERATING EXITED THE HAUL ROAD, RAN THROUGH A GUARD RAIL AND OVERTURNED. THE HAUL ROAD IS LOCATED BETWEEN WV STATE ROUTES 90 & 93. MSHA DID NOT INVESTIGATE THE ACCIDENT AT THE TIME IT OCCURRED. IT WAS BELIEVED MSHA DID NOT HAVE JURISDICTION OVER THE ROAD.
EE and Another EE responded to a stuck pallet alarm on Unit IV cooler. He became trapped when the cooler pallet released on its own causing the dump arm and wheel to swing up and strike him in the head, pinning him between the dump arm wheel and the rail.
The victim was being task trained to operator a haul truck and lost control of the truck while backing down a ramp. The truck ran through a berm, overturned, and fell approx. 30 feet to the bench below.
The victim was demonstrating the features of the fault wizard. During the demonstration in the mine office he received an electrical shock and collapsed.
Employee had hooked cable to container, lifted up, into grooves of hopper. He was making a sweep with the blade when the container popped out of grooves and swung to the right where employee was operating controls.
Injured suffered a right inguinal hernia while helping a millwright re-install a 2'x2' door on a pellet screen chute.
Employee was adjusting belts on blower motor on the No. 2 Roofbolter and placed hand on belt to ck tension.
Injured equipment operator was helping mechanic change ripper boot on dozer when he felt sharp pain in groin. This accident did not involve crusher equipment.
Employee was operating an IT-14 front end loader on the quarry upper access road when the loader rolled and ejected him and landed on his legs. It is believed that he was operating the machine with the door open and no seat belt. His legs were crushed and he lost one above the knee and one below.
Employee operating a lathe had lifted guard to tighten chuck. Left key in chuck and turned on machine key caught ip of employee s finger and cut it off.
SUPERVISOR STAGING LINERS FOR THE FINISH MILL WHEN LEFT HAND MIDDLE FINGER GOT SMASHED BETWEEN LINER AND STEEL PIPE. MINER RECEIVED STITCHES. PROJECT COMPLETED 11/18/05. 11/28/05 miner returned to dr for stitch removal when dr decided to amputate tip of left hand middle finger. On 11/29/05 tip removed. No work was scheduled for miner. Miner working at a project as of 12/4/05.
Material in a cone discharge conveyor tailpully caused the belt to stop, the plant operator radioed ee to check out the belt. He climbed up on the plant grabed the drive belts to turn the conveyor backword by hand to clean the tailpulley out. As he did the plant operator started the conveyor.His thumb went arond the Vee Belt Pulley severing the tip of his thumb.
Employee was pulling out air hose (on reel), hose caught on something and stuck. Employee felt pain in his abdomen at this time.
Employee was operating the dryer plant when QC requested a sample. Employee went to get a sample from 4"" dia. pipe designated in the sampling plant. He was unable to obtain a sample. He removed a 1"" diameter bolt, put his right index finger up into a moving screw; his finger was taken off to the first joint by the screw.
EE suffered a severe finger inury at 2:30 pm while operating a barrel trimming machine. He wipe away debris from the machine was operating to wipe the tile clean of debris. He failed to turn the rotary blade off. Blade struck his left ring and middle fingers. removal of approx. half inch of the left ring finger and approx. one inch of the left middle finger.
After completing his pre shift. He left the parking area heading for the robinson north coal pit. He noticed the driver side door was not shut. He open the doors and attempted to re-shut it. The truck came into contant with a rough patch in the road causing the door to slam shut on his right hand.
Employee reached into conveyor belt area while equipment was running. Employees hand was caught in tail pulley. Left arm was amputated approximately 4"" below elbow.
HE WAS HELPING INSTALL RIB PINS ALONG #1 ENTRY, THE DRILL STEEL AND HEAD WERE NOT ALIGNED. THE DRILL OPERATOR MOVED THE HEAD AND HIS LEFT RING FINGER WAS CAUGHT BETWEEN THE STEEL AND THE DRILL MAST CAUSING THE AMPUTATION OF THE TIP OF HIS LEFT RING FINGER.
EE was dismounting a haul truck. When he stepped to the ground from the last step of the ladder he stepped on a rock and turned his ankle.
Employee was loading ""J"" channels when he felt pain in his back.

WHILE CARRYING A PART (APPROX. 100 LB.) UP A LADDER, EMPLOYEE TURNED AND TWISTED HIS LEFT KNEE. THIS INCIDENT WAS NOT REPORTED UNTIL 12/2/05.
Miner was descending a ladder after patching holes in a chute. Ladder shifted causing miner to fall.
They were cutting long piece of steel - cut piece of metal 100"" - fell on foot. He was holding with another employee; a third employee was cutting.
He was bolting top; a rock came down drill steel, hit him on shoulder and right foot.
HE WAS HELPING NAIL TIN TO THE ROOF, WHEN A PIECE OF ROCK FELL OUT, HITTING HIM IN THE CHEST AND LANDED ON HIS RIGHT ANKLE.
Employee was installing steering cylinder on a Komatsu WA-600. Employee had placed ram end in back half of machine and was in the process of pushing up on front end to align it in position. While pushing on cylinder employee felt sharp pain in left shoulder. Original diagnosis on 12/5/05 was shoulder strain, then on 1/10/06 after MRI diagnosed as rotator cuff tear.
EE was preparing for an up coming job while lifting elevator buckets on to a chain he strained his shoulders. EE was swing the buckets onto the chain.
The employee fell from an unstable ladder and tore ligaments in ankle. Employee is currently off work.
Employee was on equipment inspection prior to start up of crusher. Slipped/tripped on the steps of the primary belt and broke right ankle. Cleanup was in progress at the time of the accident to clear the light dust and sparse rock.
Employee was pouring muriatic acid from 5 gallon pail into 1000 gallon open tank. Employee's foot slipped and his safety glasses slid to the end of his nose resulting in acid splashing into right eye.
On 3/5/05 employee was setting props in old 3 track entry, he slipped on loose slag (gravel) and fell and hit on left hip. Employee started losing time again on 11/30/2005 due to increased pain and nerve blocks.
There was an accident involving two haul trucks. Employee sought medical attention and was taken off of work.
Employee was drilling a hole in a 2"" x 2"" x 3/8"" x 10' piece of angle iron. The end of the angle iron fell off the stand it was resting on and pinched employee's finger between the angle iron and the drill press base.
Employee was moving the power cable feeding the 1300 BE dragline in order for the machine to walk back on the bench. He bent over to hook the cable to the machine & strained his lower back. *Did not go to Dr. or loose time until 12/02/05.
Employee reported he slipped on steps of a belt crossover falling and landing on hip and lower back and was in pain. Sent to dr. reported bruise
Attempting to straighten bolt, bolt flexed striking and cutting finger.
Employee had cleaned off land barge; once exiting the barge by ladder, ladder hooks broke while he was on the fifth step causing him to fall backwards.
Injured was turning handle on a winch when the handle came out of his hand and reversed direction, striking him on the right arm.
EE WAS IN THE PROCESS OF ROOF BOLTING PROCEDURES IN THE #4 ENTRY ON COMPANY #001 SEC. HE HAD JUST FINISHED DRILLING HOLE TO INSTALL A 6' RESIN GROUTED ROD & WAS IN THE PROCESS OF REMOVING THE DRILL STEEL FROM THE HOLE WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE LEFT LEG ABOVE THE ANKLE. THE ROCK MEASURED APPROX. 39""L X 24""W X 1/2"" TO 3 1/2""THICK. FRACTURED TIBIA & FIBULA.
This report is submitted under protest. Man stated that the slope car came to a stop part way down the slope when he reached for an object near his feet and experienced pain in his lower/mid back and neck. He came outside after 4:00 pm. The reported event does not support the alleged injury. The accident investigation is ongoing & is not complete as of this date - 12/20/05.
Snowy weather, windy; wet gloves. Jackhammer slipped out of hand falling over onto right foot. Bruised; took 2 days rest.
The dozer trammed too close to the edge of the spoil bank. The spoil bank failed and the dozer dropped over a steep embankment about 30 ft high.

Injured employee slipped on ice/snow while walking across parking lot from break trailer to plant entry door. Employee did NOT fall to ground but twisted foot in the course of re-gaining his balance resulting in fracture to small bone in left forefoot.
Employee was using a pry bar in an attempt to remove a final drive when he felt something give in his back.
Driver came in 4:00 am to load his load his truck. When strapping load down, strap got caught between the pallet. He got on side to get strap when he slipped and fell and injured his lower backside.
Employee was in a standing position blocking roof bolts when a piece of rock 3'x1 1/2'x6"" fell from the top striking him on the side of his left leg.
Three miners were investigating a coal feed starvation. The fine coal bin exploded cracking the fine coal bin causing a fire ball to escape which burned three miners. Two seriously and one first aid.
Employee was climbing the ladder into the back of the Euclid dump truck to unhook a chain on a grout box, slipped and his shoulder hit the grout box.
EMPLOYEE WAS HELPING RECLAIM HIGH VOLTAGE CABLE AND LOADING IT ONTO A TRAILER WHICH WAS BEING PULLED BY A 4-MAN DIESEL PERSONNEL CARRIER. EMPLOYEE WAS NOT AWARE OF HIS POSITION IN RELATION TO THE EQUIPMENT AND GOT HIS HEAD CAUGHT BETWEEN FENDER AND MINE ROOF.
Shoveling coal, he hit his hand on structure, fracturing his left hand.
Employee was shoveling coal and pushing shovel with hand and knee, possibly fracturing right hand.
Alledged incident was to have occurred while this employee was turning the cowl on the shearer. He claims a piece of rock came down and hit on top of the shield bouncing it. Cause of this injury was shield was not completely set against the roof. Compliance with rules/regs not a factor, protective equip. not a factor, miner proficiency a factor, mining systems a factor.
EE was walking to enter foreman's change room prior to the start of shift, when he slipped on ice. He grabbed a support ple to keep from falling after working his shift until 2 hrs before end of shift he went home. He was treated for lower back strain at hospital.
EMPLOYEE UNDERNEATH DRILL LOOKING FOR AN OIL LEAK, CAUSED DUST AND GRIT TO ENTER HIS LEFT EYE.
Employee was on #4 shield E-12 longwall face kneeling while pushing the panline out. The horizontal pin came out of the ram bar clevis allowing the clevis to roll catching his left lower leg between the clevis pin and clevis rail. EE sustained a fracture to the tibia and fibula of the left lower leg.
Employee was to work under 03 belt to patch up concrete but first went to get a hose to wash down the area. Trying to connect the hose to the coupling, he slipped and fell to the ground, hitting the inside of his thigh against the concrete.
Employee suffered back pain as he climbed onto his 992G loader.

WHEN IT WAS RAISED, HIS HAND WAS PINCHED BETWEEN THE PAN & THE STOP ON THE CUTTER HEAD. HIS HAND WAS BROKEN IN SEVERAL PLACES.
WHILE BOLTING TOP, THE WRENCH ON THE ROOF BOLTER BROKE AND A PIECE OF THE WRENCH FLEW OFF, STRIKING THE MEMBER ON THE RIGHT SIDE OF THE NOSE.
Busted bag stuck between conveyors. Employee tried to free it - hand got caught. Took off tips of middle and rings fingers, skin across top of hand and two fingers was pulled off. Tips reattached and skin graft to repair hand.
WHILE EE WAS PACKING BAGS OF BENTONITE THE BAG BECAME FULL WHEN THE GLUED SEAMS AT THE TOP OF THE BAG FAILED CAUSING BENTONITE DUST TO BE FORCED ONTO EMPLOYEE'S FACE, HEAD & SHOULDER. EMPLOYEE WAS WEARING SAFETY GLASSES AT THE TIME OF THE INCIDENT.
Employee was shoveling the belt and mid-shift began to feel pain in his left shoulder.
Employee was cleaning trash out of cab of haul truck when he felt something stick his finger. The next morning his finger was swollen up. Sent him to emergency room to have it checked out.
Employee was walking underground and felt his left knee pop. No specific uneven ground or other factor was noted. Physician diagnosed ACL tendon damage. Investigation determined the employee had a non-work related injury when he was kicked by a cow in the left ankle a week prior to the knee injury.
LEAKING ACETYLENE TANK INSIDE CLOSED BOX, IGNITED BY TORCH, CAUSING LID TO FLY OPEN AND BREAK ARM.
Emp was standing next to conveyor when he pointed at something and his glove was pulled in between the belt & rollers on conveyor.
EE WAS TAKING BOLTS OUT OF PLATE ON CRUSHER. HE WENT TO MOVE OUT THE THE WAY TO POSITION HIMSELF TO PULL THE PLATE. THE BOTTOM OF HIS SHOES CAUGHT ON THE BASE OF THE CRUSHER, RESULTING IN TWISTING OF LEFT KNEE. FIRST DAY OFF WORK 1/31/06.
Employee was driving a personel carrier down the MN travelway, when a small piece of rock fell from the roof and struck him in the face. This caused a laceration to the employees nose. He required 4 stitches to the bridge of the nose and 2 black eyes.
Bolting top of mine caught fingers on hydraulic hose fitting, cutting fingers.
While carrying tools to tailgate, employee slipped on flight bar and twisted right knee. EE did not start losing time until 12/3/05 due to increased pain.
Employee was removing metal plates inside the cement Mill # 12, using a mechanical jack and a hydraulic jack for the removal of the plates. When pressure was applied to the plate the hydraulic jack slipped hitting him in the upper lip.
EMPLOYEE WAS WORKING IN THE PIT AS A PUMPER WHEN THE SPOIL SLID ACROSS THE PIT PINNING DOWN THE EMPLOYEE WITH DIRT. THE DIRT PINNED THE EMPLOYEE'S LEG AGAINST A 6"" PLASTIC PIPE CAUSING A BRUISE.
Employee was struck in the nose by a pry bar he was using while repairing a jumbo drill. Employee did not experience any complications until 2 months following injury. In May of 2006 surgery was required to address subsequent complications.
EE STATED HE WAS STEPPING THROUGH A MANDOOR TO CONDUCT A PRE-SHIFT OF THE EP-22 AREA WHEN HE SPRAINED HIS RIGHT KNEE. AFTER HE SPRAINED HIS KNEE HE FELT SEVERE PAIN IN HIS KNEE AND LEG. HE TRANSPORTED HIMSELF OUTSIDE AND LEFT VIA AMBULANCE TO CHARLESTON GENERAL HOSPITAL WHERE X-RAYS WERE PERFORMED AND WERE NEGATIVE. HE WAS DIAGNOSED WITH TORN TENDONS BY HIS SPECIALIST.
Employee was sitting back plate down to re-hook, it fell on his foot and broke it.
While starting up the mill, EE slipped on ice on catwalk on his way to shut off the crusher.
On 11-23-05 employee reached for truck door and banged finger on door. On 12-13 employee went to personal physician & was given a prescription. On 12-27-05 he went back to physician and was put out of work for six weeks to prevent further dust exposure and cure finger.
EE was repalcing area differential in service truck and when he stretched to guide a part into place he strained his neck and upper back. He did not lose any time and did not seek medical treatment. He did not report injury to mine mgt. until 1/3/06. On 5/3/06 He notified mgt he would be off work beginning 5/4/06 with surgery for 2 pinched nerves and one disk replacement.
Employee was raising the bucket on a mucker to insert a metal bar under the bucket for repair work. The bar was not secured and struck him in the left jaw and left side of his head when the bucket was lowered.
THE INJURED WAS OPERATING A REMOTE CONTROLLED CONTINUOUS MINING MACHINE. HE HAD MINED THE RIGHT SIDE OF THE FACE AND WAS STEPPING THE MACHINE OVER TO MINE THE LEFT SIDE. AS HE WAS MOVING THE MACHINE, A PIECE OF DRAW ROCK FELL (BETWEEN THE RIB AND RIB BOLTS) STRIKING HIS LEFT LEG.
(LTA) : Kersey Barn - Employee was loading a nitrogen bottle in back of mantrip. Strained lower back.
Tripped on rock under belt, fell and landed on his back on the rock. Twisted back, chest and hurt right arm and right leg.
Employee climbed ladder to start his machine (hilift) then stepped across his machine on a 3 foot platform (in between machines) grabbed handrail, got shocked causing him to fall backwards between machines getting his left leg/ankle caught in ladder.
OPENED UP DOOR ON TOOL VAN AND HURT BACK.
Employee was stepping from a conveyor to a ladder on the rear of a 6' x 20' screen deck trailer. His left foot slipped from the ladder rungs and he slid down the ladder pulling muscles in his left calf and severely spraining his ankle. Further diagnostics revealed two possible small fractures to foot and ankle.
Scoop hit a stack of cribs and pinched leg between cribs and shield.
Working on slurry pump. opened door and found runner broken removed runner for further inspection and sat it on the floor a few moents later the ee put his hand on the runner to move it out of the way at that time the filing plate fell off dial pins cutting the ee s finger.
Production supervisor was preparing to lockout cement mill #2. In the process of performing the lockout, the supervisor had to lower the mill main breaker,the breaker would not lower easily, so he opened the side panel to release the breaker latch,while attempting this action an arc was created and escaped through the open panel, burning his right arm.
Injured was operating scoop tractor to clean up bottom coal when a piece of rock fell from the rib striking injured on his right shoulder and arm causing contusions.
EMPLOYEE PLACED A HAMMER ON TOP OF A BLOCK OF STONE. THE HAMMER HANDLE WAS PROTRUDING OUT OVER THE EDGE OF THE BLOCK. WHILE MOVING ANOTHER PIECE OF STONE INTO POSITION ALONGSIDE THE BLOCK, THE STONE BEING MOVED CAME DOWN ON THE HAMMER HANDLE CAUSING THE HAMMER TO FLIP UPWARD STRIKING HIM ON THE HEAD.
Employee was off loading a shuttle car axle from the scoop bucket. He was standing on level ground and using a pry bar to roll the axle. When the bar slipped the employee had pain in his lower back. He was taken to the hospital and treated for lower back pain.
Employee was driving spad into coal rib when a piece of rock fell hitting him in crook of right arm and driving elbow into mine floor.

EE was roof bolting when a piece of rock approx. 6' long x 2 1/2' wide x 4"" thick fell from the roof between roof bolts and struck him on the right leg at the calf. He was transported tot he ER medical center

Walking back to his motor stepped on a lump of coal, twisting left knee.
Injury occurred while this employee was swinging the bolter head in to drill the inside bolt hole. He slipped on a piece of rock and landed on another hurting his ribs. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equipment not a factor, miner proficiency a factor, mining equipment/systems not a factor.
Operating a Case farm tractor & disk, disking clag. Turned out of the clay raising the disk, turned in the seat to look back at the disk & drove over a rough area causing him to turn & twist in the seat. *Note: Seatbelt was being used.
Injured was operating roof bolter installing roof bolts when drill steel hit him on his left forearm. resulting in a fracture to forearm.
Member was installing a 10' cable bolt. The bolt bent and he was trying to get the bolt loose from the drill pot. As he was doing this he accidentally hit the rotation causing the bolt to spin and hit his left hand. First day away from work was 11/28/05.
EE stated that he and another ee were replacing the splash sheild to move it into position to be bolted. The pry bar slipped and/or the injured ee lost his grip on the bar causing him to fall backward striking his wrist on the floor and the apex that had been removed from the b-3.
EE was assisting with cutting plastic bags of bird balls open & dumping them into a loader. Sometime during the day he strained his back. He reported it 11/21/05 and initially refused medical care, however he eventually went to his own doctor but did not want to report it because he has a prior history of back problems.
Injured had just installed a roof bolt and as he was reaching to remove his wrench a piece of rock measuring 18""x6""x4"" thick fell and struck injured on both the right hand and right wrist.

He was trying to take out a screw in an elevator. The only way to take it out was using a hammer. When he finally took out the screw, he did not follow the safety standard and he got punctured in his left hand.

Employee was cleaning windows at end of shift. He was standing up next to driver's seat when his foot slipped and his weight shifted to his left leg. This injury required advanced medical treatment making this a reportable injury.
The employee was cleaning a floor in a service pit area and became dizzy and fell straining his back, leg and shoulder. The employee had been suffering dizzy spells for several days prior to this incident. The employee is also seeking medical treatment for the dizziness.

Employee was lifting a pump over the belt in #3 Entry, in Main A, beside #2 Head. While lifting the pump, he was swinging it over the belt, his feet became stuck in the mud - just under his knees. As a result of lifting and swinging pump over the belt and having his feet stuck in the mud, he had severe lower back pain that increased as the day went on.

Employee was operating a 777D rock truck when a loader put a large rock in the bed of his truck. The rock was off center, which caused the hauler to rock sideways. EE hit his left shoulder on the door post.
DUMPING A LOAD, THE CAB WENT UP IN THE AIR WHEN THE LOAD WAS DUMPED, THE CAB THEN CAME DOWN HARD & JARRED HIS BACK.

The injured installed a used battery on a 6"" water pump but did not clean/remove corresion from the battery posts. a co-worker turned the key to start the pump. At that time an arc occurred and the battery top failed and the battery cap and other debris flew up and struck him.
Employee has moving rocks into a bucket. One of the rocks cracked on a roller. As the employee has picking up the cracked rock, it broke in half and fell on the top of his left foot. The rock hit just behind his steel toe. There was an abrasion and slight swelling.
Employee was rebolting a rehab area when a piece of stone fell between roof bolts (rib side) striking him on the left lower lip.

Employee was in the process of drilling a roof bolt hole when a rib rolled out and struck him. He continued to work and started missing work on 12/19/05.
As EE was using a grinding wheel to remove paint and rust from skip he turned his head to look ahead of his work, a particle flew in the side of his safety glasses and lodged in his eye. It did not bother him until the next morning. It was removed by his dr.
EMPLOYEE WAS WORKING ON FEEDER AND WHEN HE PUSHED A LEVER, A HYDRAULIC HOSE BROKE AND HIT EMPLOYEE IN HIS RIGHT EYE.
While climbing down the ladder of the loader employee slipped off a rung. He fell backward striking his back and head.

EE stated his partner was welding a patch, it slipped out of his hand and hit him in the mouth.
Employee was changing shield hose and hit left arm on shield leg. He did not start missing work until 11-21-05.

His hand was flexed backward as it bent down past the side of the drill head. He had sound checked the area of the roof prior to drilling. Injured was wearing metacarpal gloves and forearm guards. Employee stated that while attempting to enter the operator's compartment of the #818-636 Shield Hauler his foot slipped twisting his left knee.

EE was bench drilling near a haul. While drilling a rock about 4 inches by 6 inches fell from the back and grazed his left arm. He looked up and then another rock about 6 inch x 6 inch came and fell on is quardracep muscle in his leg. He was treated from bruised muscle, he was to put ice and elevate.

THE INJURED WAS BUILDING A BRATTICE ON #1 UNIT. HE DID NOT HAVE THE CURTAIN DOWN AND NO FLAG WAS HANGING TO CURTAIN CAR DRIVER. A RAM CAR RAN THRU CURTAIN, HIT THE TRAILER OF BLOCK, HITTING THE INJURED ON LEFT LEG. THE CAR DRIVER DID SOUND HORN AND FLASH HIS LIGHT BEFORE GOING THRU CURTAIN.
Employee was performing maintenance on the belt press. He attempted to lift the end of a roll with a come-a-long when he felt a sharp pain in the right shoulder. The pain persisted the rest of the shift with some tingling in his right arm and hand. He went to the chiropractic doctor the next day.

Employee was lowering a drill cylinder to the ground when the choker cable slipped and released the cylinder. The cylinder fell on top of him and pinned him to the ground.

Employee was cleaning coal on 5th floor when he turned around & hit his head on a drain pipe.

HE SHOOK THE ROCK OFF HIS BACK & THAT CAUSED THE ROCK TO FALL HITTING HIS RIGHT FOOT AGAINST THE WATER LINE PIPE. IT WAS A ZINC WATER LINE. ROCK WAS 3 FT X 3 FT X 3 1/2 INCHES.
EE WAS IN THE PROCESS OF PULLING A LOAD OF WATER UP THE ROAD WHEN SOMETHING BROKE ON THE TRUCK CAUSING THE TRUCK TO GO BACKWARDS APPROX 40-50 FEET. THE TRUCK CAB TURNED OVER ON ITS LEFT SIDE. EMPLOYEE RECEIVED 5 STITCHES TO THE LEFT SIDE OF HIS HEAD. HE WAS TAKEN TO HAZARD HRH WHERE HE WAS TREATED AND RELEASED.
Employee stated that he was stepping into his motor and his fooot slipped off the step. The employee landed on his right foot causing pain in his foot and knee. Employee is a weekend worker and did not report off until 11-18-05.

Employee reported his injury on 11/4/05. His back had been hurting him for a while. He had not been wearing a back belt at work. After a few days he started to hear his back belt. The back pain persisted and he decided to report the injury and request a doctor;s appointment.

Employee stepped on a stone to get onto excavator tracks to grease machine. He fell off the rock and fell straight down 15+ feet, rolled 10 feet more, came to stop. Other employee found him, brought him to level ground, ambulance took him to the hospital.
Twisted L. ankle while splitting stone on friday didnt notify supervisor of injury. Didn't show up for work saturday off sunday. had meeting away from working 6 hr monday, tuesday notified employee of injury.
EE claimed to have injured his back on 11/4/05 while holding a 15"" bar in place against the #4 belt. EE received medical attention and eventually surgery before notifying management on 12/13/05 that it was work-related. Employer and insurance administrator determined to accept claim on 1/24/06. EE had identical injury in 2005 which was not work-related.

Employee claimed rocking motion of pickup cuased him to bump rib area on fender. Pickup was not in operation at time of incident. Did not seek treatment until 11-6-05. Reported to company on 11/7/05.
While binding a trash tub, using a cheater pipe, pipe slipped off binder arm and struck him on his face and head. Broke nose, chipped tooth, sutures required inside bottom lip.
Employee reported chest muscle pain after train derailment. He was taken off work for one shift by the attending physician.
Employee was working in construction area. Piece of metal fell off catwalk, bounced off ground and struck employee. Employee didn't feel he needed to go to doctor at this point. First medical visit 1/30/06.
Employee was cutting a head off a bolt with a torch. The head of the bolt rolled down conveyor belt and came into contact with his left knee. Employee failed to report the injury until a week later when infection had set in and he was forced to go to the emergency room for care. We found out about the injury on 11/10.
The #1 mantrip was parked at end of track where men had unloaded. The park brake leaked off. The #1 mantrip ran app 800' down track rounding a curve where it met #3 mantrip traveling toward section with a crew of men. The mantrips struck each other. The impact caused a whiplash effect on men riding in #3 causing neck and back pain.
INJURED STRAINED HER NECK WHEN THE LOADER BUCKET ARMS HIT THE TRUCK BOX OF THE TRUCK SHE WAS DRIVING. THE LOADER OPERATOR LOWERED THE ARMS TO STOP HER FROM BACKING INTO THE LOADER.
