At shift change the service hoist went down no personnel were in the mine. The drive control power and the track limit switch in the mine were repaired and the hoist was put back into operation

At approx 2:30 pm on 8/30/13, the mine examiner found a roof fall. The roof fall is located in the #3 entry of the 1-West Mains at survey station # 2700. The roof fall is approx 18' wide x 30' long x 6'-8' high. There are no injuries associated with the roof fall. The roof fall will be dangered off. Ventilation has not been affected. The roof fall was reported to the MSHA Hot-line
This report is being submitted under protest to citation #8051834. On 8/30/13 an alleged entrapment occurred at the McElroy Prep Plant when an employee was in a chute with clean coal around the lower half of his body for an undetermined time period. The inspector determined the time to be greater than 30 minutes.
The elevator was stopped as a result of a safety switch. It was unable to be reset because the elevator was not communicating with the PLC (electrically)
A fall of the roof above the anchorage was discovered in crosscut 14-28 between the track and belt. The dimensions are 53' long X 16' wide X 12' high.
Electrican had worked on the PLC on Day shift giving the #2 skip a false indication that it was full and needing dumped. There was limited use of the controller after the electrican had worked on the PLC.
Rock fall above anchor on the section submains belt #5 entry inby spad #4 4702 approximately 20ft wide and 30ft long.
A ROOF FALL OCCURRED AT 31 CROSSCUT OF THE 2 RIGHT SECTION IN THE #3 ENTRY. NO INJURIES RESULTED BUT PARTS OF THE LOADING MACHINE AND A SHUTTLE CAR WERE COVERED WITH MATERIAL. THE FALL WAS JUST ABOVE THE ANCHORING POINT. THE FALL AREA MEASURED APPROX. 28 FT. IN LENGTH AND 19 FT. IN WIDTH WHICH ENCOMPASSED THE ENTIRE WIDTH OF ENTRY AND 3 FT. INTO CROSSCUT 3 TO 2.
The relay switch at the East Finley elevator was changed out.
A roof fall has occurred in the 6th right panel, #36 crosscut, #6 entry. The fall measured approximately 80 feet in length x 19 feet in width x 10 feet in thickness. The area was supported with 5 foot fully grouted roof bolts.
Several unplanned roof falls has occurred in the Nos. 2,5,6,7,and 8 intersections one crosscut inby survey spad No. 375 the mine is in BA status and no one working underground only weekly examinations being conducted. All access to the falls will be dangered off and cribs built to prevent entry of persons.
Lost control power to PLC for automatics. Disconnected automatic control cable allowing hot to operate in manual mode. All miners evacuated safely with no injuries or other issues. All employees returned back underground at approximately 8:30 AM.
D11T dozer ruptured hose and a fire started underneath the engine, under floor panels and around the transmission.
An unintentional roof fall has occurred in front of the #1 seal at the Route 1 Seals xc 27 entry #9 (6th 48 Main Return). We are not planning on cleaning it up, we are going to add 3 seals around fall. This area was supported with 72"" tension rebar fully grouted roof bolts and additional cribs, props and timbers were added to the affected area.
Longwall Panel 61 experienced adverse roof conditions at the headgate which impeded passage to the primary escapeway. There was no roof fall.
4703 Service Hoist had a failure of the 24 volt power system on the right hand brake of this hoist. Electrical component was replaced putting the hoist back in use.
Machine break fault was activated. No men trapped on the elevator. Elevator was back in service at 11:30 a.n.
Dry Ridge elevator stopped moving in the shaft with an employee on board. The employee was not in any danger but was entraped for about 45min
There was an unplanned roof fall in #4 entry of the 2nd Main East at 920' mark.
A highwall failure occurred in the Stockton pit #24 area of Tower Mt Surface believed to be caused by subsidence from previous underground mine works in the coal burg seam below. No injuries resulted from the event.
At approximately 7:00 p.m. the slope hoist experienced an electrical malfunction which prevented it from functioning normally.
The sensor for the main brake location malfunctioned and was not communicating with the hoist PLC
While kiln #2 was in the cool-off stage of a maintenance shut down, pulverized coal was inadvertently injected into the kiln. When the kiln was turned to prevent warping, the coal ignited resulting in an immediate release of energy.
A fire occurred on the 20520 D10 T Cat Dozer. The fire was extinguished after 35 minutes. No injuries occurred. MSHA was notified of the occurrence.
An unplanned roof fall occurred in an intersection in the primary escape way intake #6 entry spad number 3022. The fall measured app. 40'long X 70'wide X 6'high.
2 fire watchers armed with extinguishers didn't see signs of fire during hot work. after saw flame 30' below on rubber screen, attempted to trigger the process water to the plant - not turned on, tried extinguishers - no help. called fire department. 2 screens, motor & electric wires burned.
Roof fall was discovered during weekly examination. The fall was outby the working section in the #1 entry. The roof fall was approximately 100 ft long and 5-8 feet high.
An unplanned roof fall had occurred at the Road Fork #51 mine 1 break to the north of spad #220 that measured approximately 20 feet by 20 feet by 5 feet thick. No injuries were reported and additional supplemental support was added to help make the area safe.
An unintentional roof fall (30E) occurred in the 10WHG footage marker (approximately) 7295, xcut 51, between #2 and #3 entries. The dimensions of the roof fall are 10'T x 18'W x 16'L. The fall will not be cleaned up.
A fall of roof has occurred in the 6th right panel, # 34 crosscut, # 5 entry. The fall measured approximately 25 feet in length x 25 feet wide x 12- 15 feet thick. The area was supported with 5 foot fully grouted roof bolts and 8 foot fully grouted roof bolts.
Breaker failure at the hoist on the 1085 level
Hot ember from cutting torch smoldered on a urethane screen panel. Decks caught on fire in #4 screen doing damage to the screens and chute work. No injuries to any employees.
The headgate area on the 7B Longwall at spad #22+65 took weight and affected the air on the Longwall face. The area that took weight was approximately 25 feet long X 9 feet wide. Not a fall above the anchorage.
#9 elevator was inoperable due to the machine break fault. No men on the elevator at the time. Elevator was put back into service at 3:00a.m.
While bolting in U8 E tunnel 53 panel a small pocket of methane was breached and bled out within 20 minutes highest reading obtained was .9 percent.
On 8-19-13 while operating a shaft sinking operation at the Pumpkin Hollow Mine. At 7:25PM the hoist experienced a loss of power due to an electrical storm. Miners were evacuated using the Grove 890rt crane with manbasket. After the electrical dept. trouble shot & reset the hoist. The hoist resumed normal operations at 9:30 PM. MSHA was notified via the 1-800 Number at at 8:05 PM.
A non-injury roof fall occurred on 004mmu in #4 entry approximately 106 feet inby spad#4962 during retreat mining the roof fall on the continuous miner they were taking the last lift from the left pillar block
An unintentional roof fall occurred in the 10WHG footage marker (approximately) 7285, xcut 51, #2 entry. The dimensions of the roof fall are 18'W x 10'T x 30'L. The fall will be cleaned up and a roof control addendum has been submitted and is currently under MSHA review.
The #1 elevator lost control power at 10:55 p.m. EE, an electrician was dispatched to the #1 elevator and troubleshoot the issue. He did not find any defective equipment or components during his troubleshooting phase. Power was reapplied and the elevator was tested, it was returned to service at 12:45 a.m. The elevator was down for one hour and fifty minutes.
A reportable roof fall was found in E South Mains, entry 6, crosscut 28. The fall was approximately 10' high, 20' wide and 25' long.
40ton Volvo dump truck was waiting for an excavator to pass. Excavator cab was cocked to the side and slammed into the front left corner of the truck. Would have been avoided if the cab was sitting square with the tracks. The corner was sheared apart in a 6 inch tear and hydraulic fluid spilled about 2 or 3 gallons. The tear was able to be welded shut.
Shaft conveyance was being lowered into the mine, transporting personnel. A sound was heard which was reported to the shaft crew supervisor. A guide wear plate caught the upper lip of the steel shaft guide which rolled back and peeled off, approx. 6""X6"" piece. This needed to be repaired before the shaft could be returned to operation. The mine was evacuated during the repairs.
At approximately 4:45am the Schindler elevator became inoperable. Electrical components were changed out and the Schindler elevator was again in operation at approximately 7:15am.
A roof fall occurred at 2nd Main North 3 East. The cause was weathering. Dimension 15 ft long X 10 feet wide X 8 feet high.
At approximately 12:45pm the Slope Hoist became inoperable due to an electrical malfunction. Parts were ordered and delivered. At approximately 4:10pm the hoist was back in operation.
An unplanned roof fall occurred in the Wyoming #2 mine, 1 break outby spad #1971, in the #4 entry, that measured 7' high x 20' wide x 25' long. No injuries were reported. The area was developed on 12/1/2011.
The examiner found a roof fall at x-cut 93 in the main north in entry #7 while doing his weekly exam.
A roof fall occurred in the 1st Main South belt line between x-cut 41-42 in the pillar line #5 neutral entry. The fall meas. 35' x 6' x19.25' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension glue bolts and was mined in 8/2006. There were no injuries and no equipment involved and ventilation was not affected.
The Dry Ridge Elevator was inoperable for an extended period of time. No one was on the elevator at the time of the incident. It was found that the door sensor on the outside door malfunctioned. The sensor was adjusted.
A fall occurred at #58 Block on the #6 Belt in the #4 Heading. The fall area measured 20' high x 14' wide x 18' long. The area was bolted with 6' tension bolts and 18' cable bolts. It was later rebolted with the same type of bolts. No personnel, ventilation, or equipment was affected.
The K3 elevator was descending to the ground level and stopped between the third and second floors. Executive elevator was called to troubleshoot and reset the elevator and get the contractor employees out.

Crew has just mined 15' out of #5 Entry off 20x10 x-cuts when roof fell in cut, then intersection started working. They backed the miner up and a few minutes later the intersection fell in.
A roof fall occurred in the #5 entry spad #3+70 along the 1st south sub main off the sturgis mains. The fall measured approx 18'7"" wide by 30' long by 12' plus high.
An unintentional roof fall occurred at or near spad# 2488 thru spad# 2489 on the 1st south east mains. It measured approx. 8'h x 20'w x 100'L. The area is now sealed.
A roof fall was discovered in the 19-W section in the # 3 entry outby 87 block above the anchorage bolts. This fall was a continuation of a recent fall that was supported. There were no injuries, ventilation not disrupted but the walkway is obstructed. This area will be resupported and walkway reestablished.
Found an unintentional roof fall on the # 8 Belt(Right Fork Mains) between breaks # 24 and 25. Fall dimensions are 40' long by 20' wide by 7' thick.
Found an unintentional fall in breaks #23 and #25 on the Leslie Perry Mains in the #7 entry. Fall dimensions are 130' long by 20' wide by 7' thick.
A coal/rock outburst occurred while retreat mining on this date. The outburst occurred on 6-Left Panel off B-4 Mains inby Break #35 in the #3 entry. There was one fatality and one serious injury related to this outburst which have been previously submitted on submissions # 1769828 and #1769459.
On 8/6/13 while operating a shaft sinking operator at the mine the hoist experienced a fault on over and under wind of the hoist control panel. The fault would not let the hoistman operate the hoist The electrical dept. trouble shot the fault reset and tested the hoist. The miners in the shaft exited the mine via secondary escape using the 890 RT crane and man basket.
A non-injury, non-fatal roof fall occurred at this operation on 8/6/13 in the 1 North Main-Left side intake. The fall was discovered by MSHA inspector and company mine examiner during weekly exams. The fall measures approximately 8' thick 100' long and 18' to 20' wide.

A roof fall was discovered in the South Mains located at break number 5. The fall was approximately 20' long and 5' thick at the highest point. The fall occurred in number 6 entry. The fall was dangered off.
Found an unintentional roof fall between breaks #26 and 27 on the Leslie Perry Mains in the #6 entry. Fall dimensions are 80' long by 20' wide by 7' thick.
There was an unplanned roof fall at the # 14, 1st north main return, cross cut 19.
A roof fall was discovered at 7th North West Submain (4+50) xc#5, entry #4. Approx 22'L x 18.5'W x 6'+H
An unintentional roof fall has occurred at xc 3 on the 8th 48 travelway, entry #6. We are not cleaning this fall up, we stepped the travelway over. The area that fell was approximately 20' x 20' and was bolted with 72"" tension rebar roof bolts. The affected area was re-supported with 84"" bolts, brow bars, cribs, props and timbers.
A fire on the Load center in the E Mains section was reported at 5:40 am by a fireboss. The fire was extinguished using fire hose on a beltline fire tap, a fire car, and smothering with rock dust.
A roof fall occurred in the C4 Longwall section in the # 7 crosscut 2-3 entry. The fall measured 12 to 14 feet above the anchorage point.
Roof fall occurred in an intersection in the R-3 entry (Intake) of the 12-Butt off 2-Right Continuous Haulage Section approximately 310-feet outby the working face. The area had been supported using Jenmar 6-foot ""Suprtwist"" roof bolts with 8-inch x 8"" plates on 4-foot x 4-foot centers. No roof defects noted.
While conducting his weekly airway examines the examiner identified a fall in old 3Lt.Panel. The fall was located outby spad 9895#2 intersection & fell up the #2 entry for approx.120'then Lt. handed thru CC into #1 & continued outby for 100'. The fall was 20'WX220'LX4-5'H supported w/4'fully grouted. Area will not be cleaned up has been dangered off and crib & timbers set.
A roof fall above the anchorage zone occurred in entry #5, crosscut #12, on the 1st 42nd Southwest conveyor belt line on 8/1/13 at approximately 22:40. The fall was approximately 25 feet in length, 12 feet in height, and all openings measured 17 to 18 feet in width.
Slope hoist was down due to wire came off stop switch.
The technician was assisting in moving a 2800 P&H shovel when the machine lost propel capabilities. The machine then rolled in reverse striking the pickup driven by the technician causing fatal injuries.
While conducting retreat mining on 6-Left Panel off B-4 Mains, a coal/rock outburst occurred inby break #35. The outburst involved the continuous miner operator who suffered fatal blunt force injuries to his head, chest, and legs.
Employee was looking into Pegson stone hopper when the missing excavator tooth was ejected striking employee in the head.
Employee states his hand was outside of the cab of the scoop, and when turning 3 of his fingers were caught between the cab of the scoop and the rear section of the scoop. Compliance with rules and regulations, involvement with mine equipment or mine systems, or protective items are not a factor. Job skills and miner' s proficiency may be a factor.

EE was mounting tire back on unit. EE used a push bar to set flange and lock ring. While putting the lock ring in place the push bar slipped and the tire flexed which struck the index finger on the left hand and the middle finger on the right hand.
Delivery driver mashed hand between the lift gate of delivery truck and a 55 gallon drum of antifreeze.
The driller was changing a drill steel when he noticed the steel was cross threaded on the power head. He pushed on the steel with his rt hand to correct the problem & the steel suddenly disconnected from power head & dropped onto the table, crushing the tips of his 2nd & 3rd fingers on his lt hand which he had rested on the table. The 1st joint of his ring finger was amputated.
Was going to adjust the bearings to make sure nothing was stuck in the pulley. Door of tail pulley mag conveyor was stuck. He was using a 6' long pry bar to pry open the door that covers the bearing assembly. Severed right index finger between nail and first knuckle on edge of conveyor.
Employee was cleaning a pile of material under a screw conveyor. He noticed a leak in the conveyor housing and attempted to stop the leak by inserting a rag into the hole. The moving screw inside the housing amputated the tips of his right index and middle fingers just below the fingernails.
Original Pain was felt while moving a Nitrous Oxide bottle. Pain worsened while lifting coal sample out of the back of a truck.
Stated: Employee was in XC77, 5L, watching FCT cable while mechanics were pulling up tail of lo framing. He was on one knee when the inby corner of the pillar broke off and struck his back and head. A mechanic heard him call for help. When he was on the surface he indicated he could not feel his feet, that was the first indication that his head was not the primary concern.
Preparation laborer was cleaning with a water hose when his leg became wrapped by the hose. He took a step forward he lost his balance and fell backward striking the back of his skull against a concrete floor.
Contractor was helping in the installation of water line. He was asked to get components for the water line, contractor turned around and slipped and then felt pain in his back.
Employee was asked to hang 30' feeder cable on the rib mesh. This is usually a two person job and employee did this alone. After lunch he proceeded to throw dust in the LOXC and pain worsen.
Bent down to throw a track switch in the supply yard and the switch throw hung up so he used his right leg and foot to push it down and felt a pain in the groin area. He was seen by a physician and diagnosed with a hernia. He began missing time on 9/3/13.
EE was working as a section foreman and was in the process of hanging a set of fly pads between #5 and #6 entry located beside spad#3625. While hanging the fly pads EE fell from the ladder striking the back of his head resulting in a concussion.
Injured employee sustained a fractured right hand while operating a single head roof bolter on 1 North Mains. While drilling a hole he lost suction on the bolter. Injured employee lowered the drill head and placed his hand on the drill head to check for suction when the drill steel struck fell from the top and struck his right hand.
Employee felt pain in right shoulder while throwing timbers through door. Started losing time on 9/19/2013.
EE was spotting belt to make splice when he spotted a flapper on the belt. He stopped belt and started to trim flapper with a belt knife. He pulled the knife toward his leg and when the knife cut through the belt, it went into his right inner leg just above the knee. He cut two blood vessels in his leg. The wound required 6 stitches to close.
Injured employee was exiting the cab of a diesel scoop when he twisted his right knee on the mine floor. He continued to work up until September 13th when he was taken off work for a torn meniscus in his right knee.
The rope was not properly placed on the dock cleat. This caused the rope to bite or stall. At this point the employee kicked the rope free, there was no way to hold the tension back on the rope causing the rope to slap upward and hit the employee in the hand, resulting in the injury.
Employee was riding in a locomotive when the cars uncoupled and a safety chain pin broke striking the employee on the cheek. 5 stitches.
Employee stated that he strained his lower back when bent over to pick up a rock to mark drill pattern. Employee did not begin missing work until 10/10/13. The lack of medical records caused the slight reporting delay.
Stone was being lifted out of a bin using a crane and sling. The employee was placing the sling around the stone to be lifted. When the stone began to be lifted, it slipped out of the sling and rolled into the employee's left knee causing a patella fracture.
The employee was hanging the roof bolter cable from entry #3 to #4. While reaching above his head to tie the cable up he felt a sharp pain in his back.
The employee was putting a drill steel in when a piece of top fell out and struck his right thumb.
Employee was walking down the steps from the 6th floor to the 5th when he felt his right knee pop.
EE was walking down the slope beltline and slipped and fell, causing pain to his lower back. He was seen by his family DR. on 9/3/2013 who took him off work until his next Dr. visit.
Standing next to bin loading a truck with slate and something flew in to his eye
The connecting band on a drain pipe came loose and struck employee in left side of face on his cheek.
Employees were dislodging 3"" rock from tail pulley on conveyor. Co-worker engaged lever for conveyor to start as EE reached into conveyor causing his left arm to become stuck for approx. 30 to 40 minutes. EE was transported to the hospital where he received stitches to re-attach bicep muscle. All employees have been refreshed on lock out tag out.
WAS RETRIEVING A ROOF STRAP FROM TOP OF MINER WHEN THE SUMPING MINER SQUEEZED HIS FOOT BETWEEN THE BOOM AND MINER FRAME. STARTED MISSING WORK ON08/29/2013.
EE RUSHING UNDER CONVEYOR TO AVOID GETTING WET & HIT HEAD ON CONVEYOR FRAMEWORK LABELED LOW OVERHEAD. EE REFUSED MEDICAL ATTN AT TIME OF ACCIDENT. EE ON VACATION 8/28-9/2/13. ON 9/3, HE COMPLAINED TO MGR OF STIFF NECK/DIZZINESS. EE SENT TO CLINIC ON 9/5 & INCIDENT BECAME REPORTABLE DUE TO LOST TIME.
Employee was climbing the ladder on 988 Loader and his foot slipped and he held onto railing with left hand to prevent falling. He heard a popping sound and felt pain in his left shoulder.
Refiner was pulling a precip tray out of the retort and strained his lower abdominal muscle. Employee didn't report injury until 9/13/13 and was referred for medical treatment on 9/16 and was subsequently placed on modified work.
Employee was changing a truck tire in the shop when he felt a pain in his groin area. Employee did not seek medical repair until 10/09/2013.
EE was putting a new ball stud in a sterring jack, While striking it with a hammer a piece of metal flew off and hit him in the left eye. EE was taken to PMC Hospital immediately after the accident, and then was transported to Hospital in Morgantown.
A conveyor belt had been set on the ground by lifting equipment, the employee went onto the belt of the conveyor to unhook the lifting chains, the lifting equipment subsequently bumped the conveyor while the employee was till on the conveyor belt, and the employee fell onto the ground.
While straightening out a section of roof mesh, the employee's right foot slipped off a step on the left side of the miner bolter resulting in a laceration of the scalp requiring 14 staples.
Kneeling near sheet metal that was being loaded by forklift, metal slipped off of forks and hit employee on top of left foot causing a fracture
While spotting dust out of a dust car he felt his back strain
While walking around the machine that crushed stone I feel a pain in the bone. I could walk.
8-27-13 employee did his walk-out from 401 to XC10 and his back started hurting. On 8-26-13 he shoveled for 75% of his shift and other 25% was spent carrying wooden crib blocks (oak) to HG & last open cross cut.
While unloading rock dust off of a supply car, the car ran over his foot.
As employee was exiting the bonnie car he was crossing at the front end of the car, he dropped his tools. As he turned to pick them up, he tripped on the bonnie car rope falling, landing on his left elbow.
Employee was tightening skirt rubber on tight side of the belt in the draw off tunnel. Employee was not aware of his surroundings and stepped off the edge of the draw off tunnel. He fell approximately 7 1/2 feet into the tripper pit, resulting in an avulsion to the right side of his head.
Employee was driving a haul truck and hit a pothole, jarring him in the cab. He had pain that did not go away. He was transported to the ER and diagnosed with a fractured rib. Put off work and told to follow up on 09/03/13.
Employee strained his lower back while reaching for a water hose.
PRE SHIFT START TIME EMPLOYEE GETTING STUFF OUT OF HIS VEHICLE AND WAS STANDING TO CLOSE TO DITCH BERM AND DITCH GAVE WAY AND EMPLOYEE TUMBLED INTO DITCH. FIRST AIDE ADMINISTERED AT HOSPITAL. EMPLOYEE STATED DR. DID NOT PUT HIM OFF FROM WORK, BUT HE CAN'T COME BACK YET
Employee was exiting loader and received a shoulder injury. Employee was taken to the emergency room and is going to have surgery for a torn rotator cuff and has not returned to work.
As employee was preparing to enter the operator cab of a Wagner 25X in 22nd West, rock and coal sloughed off from the roof/rib and hit his right shoulder area. EE went to the ER, x-ray were negative, diagnosed abrasion right shoulder, released for work. The next day his neck was stiff, went back to ER and Doctor has him off from work until 9/3/2013.
He was hanging a chain for a ventilation tube and laid his hand on top of the rib protector door. The miner trammed forward and pinched his right pinky finger between the rib protector and the rib.
Employee was walking by scoop charger when he tripped on charging leeds. Twisted his right ankle.
Employee was walking around the end of a fly pad when he struck his head on the rib. He continued to work up until 1/17/14.
Employee was splitting stone. As the hammer struck the stone he rolled his wrist, re-injuring a pre-existing condition.
Employee was operating a stone cutting machine and caught his hand between a rock and the machine.
Employee was making his normal fireboss run on a ride and was coming through the outby airlock doors. He got off the rail ride to close the doors. As he went to close the doors, he noticed the ride to start moving and he ran and jumped onto the ride when his foot was caught underneath the ride which resulted in a fracture.
Injured was lifting a heavy plate with a helper felt several pops in his elbow joint causing loss of strength in lower arm. PPE was not factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, proficiency was a factor. This injured became a lost time injury on 10/3/2013, he worked everyday until 10/3/2013.
He fell backwards walking up stairs into a storage room about 4ft high. There are hand rails present. He claimed he hurt his leg and could not move it. He was taken to the hospital by ambulance and they released him about an hour later. The hospital said he would be able to return back to work 8/30/2013
Employee was helping set a water pump when the suction hose fell from the loader bucket and landed on his left foot.
Employee was walking down steps from crusher house and slipped and fell.
Employee was opening a man door in the shop wash bay. The door was jammed so the employee pulled hard on the door and felt a sharp pain in his left shoulder.
injured was reinstalling a guard the had been removed to allow cleaning under the overland belt line. injured lifted the guard up and felt tear in his right shoulder. belt was off and locked out.
While installing engine exhaust manifold and engine turbo. The technician had to work in a bent over posture for an extended period of time. No pain was felt on the day he was performing the work. The technician began noticing soreness and stiffness in his right lower back on 8/26.
As Employee walked past scoop batteries his shoulder hit the lid which was propped up. The lid fell striking Employee's right hand which caused a simple fracture to the 5th metacarpal bone.
Walking down stairs, mine was spraying water, slipped and fell on stairs.
Foot slipped off ladder rung while climbing down from 988B Front End Loader. Fell backwards and hit head, elbows and back of neck.
Employee was repairing the lining in the trough under the wash screen when he picked up a bucket of cement and strained/sprained his back.

Employee was assigned to clean up lime dust/powder off a concrete pad. He pulled a muscle in his lower back while using a shovel.
The employee was walking along the catwalk along C-7 conveyor when the last section of Gripstrut catwalk slipped out from under his feet causing him to fall about 9 feet to the ground. The section of catwalk was not bolted down.

Individual was bolting on the left side of the Alpine Voist Miner when struck by a rock from the top of rib line struck the left shoulder. Individual experienced pain to the shoulder.
Removing ventilation tubing and was struck by a falling rib 2 feet x 3 feet x 1-1/2 feet thick that struck him on the left side of his body on the shoulder, arm, leg and knee. He continued to work until 12/16/13 at which time he was taken off work for surgery to his left knee for a meniscus tear.
Employee was observing a crew setting roof support beams while his hand was lying on the continuous miner and an employee flipped a beam over and it struck his left 5th finger. Diagnosed with a fractured finger.
Employee was operating a D-9 Caterpillar Bulldozer when he backed up over a rock causing pain in back and numdness in arm.
Employee was operating a Scoop along the intake haul road, when the machine encountered a road way irregularity, which jarred the employee and caused him to strike his head on the machine canopy.
EE was installing a cleaning lip on a loader. EE hit the lip with hammer causing a metal fragment to enter his right arm. EE went to E.R. EE went to another doctor and had the object removed on 8/28/13 and missed one day of work.
Employee exited equipment improperly, not maintaining three point contact. Upon exiting employee twisted ankle. Employee was diagnosed with a distal fibular fracture and a torn tendon.
On 8-21-13 employee struck a piece of coal causing the steering wheel to spin straining his shoulder. On 8-26 he was seen by a Dr and released for work. On 9-4 he was taken off work pending an MRI. EE has previous neck and shoulder injuries and this case was under review by industrial to determine if it was a new or old injury. On 10-10 it was accepted as a new injury
Employee was tramming a Ram Car back to the Continuous Miner. The Continuous Miner has just moved to a new location between crosscuts #7 and #8 entry when the Ram Car struck the tail of the miner causing his head to strike the canopy of the Ram Car.
Employee was walking down an old track entry and cross over an old cement pad. He slipped landing on his hand and hyper extended his thumb.
Employee was driving a battery scoop when he ran over a rock in the haul road, causing him to hit his head on the canopy of the scoop.
Operator stepping out of an 18 inch trench indicated he felt his hamstring muscle pull but without pain. Did not report the injury that day. Came to work on the 22nd and indicated after working four hours to his foreman that he had injured his leg the previous day and needed it looked at.
Driver and passenger were driving down the haul road to Pete Bajo Pit from Leeville. When trying to make a left hand corner the truck tipped on the passenger side.
On August 21,2013 employee was exiting the conveyor frame when he slipped an caught himself with his arm on the handrail hurting his shoulder. He did not seek medical treatment until November 7,2013.
Employee was washing counter weight on Conv 1A. while squatting down washing under counter weight a rock fell off the conveyor and hit him in the right thigh.
Torque was being positioned under the D10R using a winch to pull the torque under the machine. A pry bar was being used to guide the component. The bar became wedged between the machine and the component causing the bar to whiplash back and strike the tech in the face.
Employee was putting up a pin when the pin board smashed his right little finger between the pin board and the bottom of the canopy.

While walking in the Agglomerator Step III, injured lost his footing and fell
While removing a tramp release cylinder from the CR02 MP1000 crusher utilizing a come along, the cylinder tipped forward coming in contact with contract employees right foot causing a contusion to the metatarsal portion of the foot.
While changing screen cloth he was using a bar to position the screen panels. The screen above slid down and struck the bar bending his wrist backward. Original diagnosis was a sprain/strain of the wrist. The employee was diagnosed with a fracture on November 27, resulting in lost time.
While helping to pull a loop of miner cable felt his stomach tighten. He continued working and didn't think anything of it, took his belt off later and did not feel anything. After he went home and was using the bathroom observed a bulge above his left groin. He completed this incident report on his next scheduled shift. After being seen by a specialist began missing work 9/9/2013.
Employee was refueling his loader and stepped on the edge of the concrete base, approximately 6 inches above ground level, severely twisting his ankle.
Injured employee was roof bolting on the right side of the dual head bolting machine and attempting to drill the inside bolt hole. The area had potted out. As he was drilling the bolt hole a piece of slate fell out of the top striking the employee's right collar bone and shoulder.
Employee was attempting to pull a metal basket out of a trailer he was loading with bentonite clay. While doing so he injured his right bicep. The basket was in bad condition and a new basket was already being contructed by a contractor. Employee should not have been using the damaged basket.
Miner operator was not aware of the events that happened while he was mining. After taking a cut of coal he had no memory of mining the cut.
Employee was manually lifting rocks from the ground and tossing them onto the conveyor. While doing this, his hand made contact with the railing, cutting his finger. He was taken to the ER, requested and was given a day off by the physician due to pain. No stitches were needed.
Employee states that a piece of draw rock broke out between bolts and struck him in the head. He completed his shift and went to the ER afterwards. He was given 1 day off. On 8-22-2013 he went back to the Dr. where he was taken off work until further notice.
Injured employee was lifting the continuous miner cable out of the hangers on the 7 Right section when he felt sharp pain in his lower back and groin area. The injured employee was diagnosed with a strained back and hernia.
He was at break 37 on 2 belt at Tool Storage station. He moved a 4 wheeler tire out of the way to get in his tool box. He felt pain in lower groin area.
Employee was pulling O2/Acetlyne hose from his truck to work destination (approx. 30ft) and felt a sharp burning pain in his left bicep.
Fixing a valve at the north end of the plant. Fell in a hole and broke two ribs and scraped elbow.

Employee was walking the 401 Belt and encountered a slippery area where water and gypsum had mixed. He felt his heel slip and tried to catch himself. As he attempted to catch himself he felt a sharp pain in his left leg and fell to the ground.
Stepped out of diesel motor (left foot on step, right foot on mine floor) and right ankle twisted inward. Went to Ruby Memorial Hospital and had a cast put on, however no fracture was diagnosed at this time.
Employee was in the process of making a belt splice when the utility knife he was using slipped and lacerated the index and middle finger on his left hand this causing him to receive stitches on both fingers.
On August 17, 2013, EE picked up a belt roller, twisted around and felt a pop in his back.
EE developed tendonitis in right wrist while stacking bags.
While changing drill steel, the employee's right hand was caught in the drill steel. Taken to ER where he was diagnosed with dislocation of the 3rd, 4th, and 5th fingers and lacerations to the 3rd and 4th fingers. Stitches required to close the lacerations. Released to restricted duty.
Lack of communication between the men. Man put hand in conveyor pinch point of chain when bridge was energized. Operator of bridge at time hadn't seen him with hand in area and the chain started when machine started. Injured person refused ambulance.
Employee was installing 6' torque tension bolts when he felt pain in his back
While stepping off a landing onto a fixed ladder, employee missed stepping on the first rung while simultaneously losing his grip on the ladder's handhold. He fell approximately 11 feet to the ground below landing on his feet.
The employee had just finished scaling down loose rib when a piece of rock fell from the rib striking his left arm/wrist causing a laceration and bruising.
Injured employee was operating the continuous miner on the 7 Right section mining coal in the crosscut 2-1. While mining, a section of the wedge cut fell from the #2 face, slid down the coal that had been cleaned to the face and struck the employee's legs knocking him to the ground resulting in a bruised/strained back.
Maintenance employee was replacing a skirt rubber on a chute above the main feed belt. The employee did not Lock Out the feed belt and while working on the chute a piece of metal fell onto the moving belt, the employee tried to grab the metal and caught his right arm between the belt and belt cover. The employee sustained a fracture to his right wrist and forearm.
Low-Boy driver was picking up a bull dozer. After climbing up onto the tracks he was in the process of trying to open the door. Before he was able to open the cab door he somehow lost his balance an fell to the ground. The driver suffered a fractured Tibia from the fall.
Employee was pulling a water pump up the hill and it slipped out of his hands. When he tried to grab it, he fell on his side and hurt his left shoulder. Employee was able to get up and walk into the office, his complaint was pain in left shoulder. He was then sent for medical exam.
MINER WAS DRILLING AND SPLITTING STONE. STONE WAS NOT SECURED AND FELL ON HIS RIGHT FOOT, PINCHING IT.

The Employee was helping his coworkers installing a 3x200 feet band on the Grisly Crusher. When he was pushing the band with his hands, the band moved fast and pushed his left hand towards a frame underneath.
Injured was loading bolt supplies and felt pain in his back. He continued working and received medical attention and returned to work on his next scheduled shift. He worked until 08/22/2013 and went off under DR care.
Injured was carrying a piece of belt structure off the rib, turned to take it to the belt stepped in a hole fell striking the back of his head to the ground causing an abrasion to his head. PPE was not a factor, mining equipment system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
IE was washing down and noticed a leak coming from a pipe. While placing a magnet over the leak, material shot out striking him in the face. IE received first aid treatment but went home and his condition worsened. Went to the ER and was diagnosed with a corneal abrasion. He went to ophthalmologist the next day, was given prescription eye drops and patched his eye.
Employee states that he carried 5 pails (5 gallon) of oil up a flight of stairs to perform a PM on a haul truck and noticed a stinging sensation in his lower back. His physician has ordered physical therapy for his lower back and taken him out of work for 4-6 weeks. He has just put us on notice of the injury. Employee did not report at the time of injury.

The employee was attempting to stand from a seated position and dislocated his left shoulder. There was not trauma incident involved, the employee states that he has a history of shoulder problems.
Employee was standing on opposite side of the Scoop from the employee using the hydraulic chain saw, when the chain on the saw broke and a small metal link from the chain struck the employee in the chin causing fracture and requiring sutures to his chin.
Employee was making a splice in the shuttle car cable and as he was striking the knocker he accidentally struck his left thumb with the hammer. This resulted in a laceration that did not require sutures.
Welding on a Joy 14-9CM when a piece of hot material went into boot causing a burn on the inside of the left calf area causing a burn. Was not reported until 8/18/13.
EE was in a hurry to get fire extinguisher from in back of his cab and slipped and fell from the cat walk. EE landed on his foot breaking bones in his ankle.
Dump truck driver was driving with box in the air for too long when the box struck the overhead conveyor, driver was thrown from truck onto ground & truck rolled to a stop approx 1/4 mile later. Driver has some compressed & cracked vertebra but is out of hospital & walking. Conveyor has bent pieces but was retrained & working within 30 min. Hoist & subframe on truck were bent.
The employee was carrying a rock dust bag and stepped on a rock twisting his left knee. The accident occurred on 8/14 became lost time on 8/31 when the employee was diagnosed with a partial tear of the MCL in his left knee.

He was administered first aid and then it was determined that an object was embedded in his arm. He was transported to the ER. He had surgery on the morning of 8/15/13 to remove a piece of metal. He was taken off work by the surgeon until his next Dr's visit.

Two welders were welding on the auger, the injured employee was helping the two welders hold the metal that they were welding. The flash from the welding burned the injured employee's eyes, resulting in his eye sight being limited.
EE was working on light tower. Two pipes were stuck together, while pulling and twisting to get them apart he hurt his back.
Employee was putting sand in the sanders on a diesel locomotive when he strained his back.

Driller was lifting a 5 gallon can of compressor oil to put into reservoir of drill during maintenance, he felt a pain in his shoulder area, went to hospital and they said it was a strain to tendon.

Employee was sitting in a chair. He leaned back. The chair tipped back. The employees chair fell back striking the back of his head on the wall where the A/C was plugged in. Employee suffered a 1 1/2 inch cut to back of head.
Employee had began installing the inside bolt when a rock fell from the roof striking him on the back of the left leg resulting in a simple fracture.
EE lifted a 15 lb garbage can to mop and felt a pull in his stomach. He was seen at a local doctor and given a full release he later went to another doctor who gave him an off work slip for a hernia. Montana State Fund questioned this claim and sent it to for independent review. This claim was accepted by MSF Sept. 6, 2013. The EE will have hernia surgery later this month.
Getting out of dump truck, driver slipped on steps and fell to ground hitting his left hip. Steps were wet due to rain.
Was climbing ladder on fuel truck to check the fuel level, when the employee felt a pinch in his right upper arm/shoulder.
Crew was loading empty rock dust sled unto a supply car using a scoop, car was parked at end of track and was being loaded from inby end of car. Scoop became stuck on supply car and employee went to check when he became caught between the car and bucket of the scoop. Accident was investigated by MSHA, Pa. DEP, GMS, and Consol Energy
Employee was hanging fly pads between #5 and #6 and a piece of top rock fell out from the rib bolt to the rib about 7 or 9 foot long and about 3 foot wide and about 3 or 4 inches thick braking the fly board and hitting employee on his left side in the head, neck, back and shoulder areas knocking him to the ground.
Employee was sitting in a 785D rock truck while it was being loaded. A larger boulder was dropped in tossing the truck from side to side causing the employee lower back pain.
P&P Contractor began hanging feeder cable when he felt a strain in his left shoulder.
Employee was standing on the platform while the ATRS was lowered to put up the last row of bolts, when the ATRS lowered a rock fell ahead of the boom and knocked the operator off the platform. He fell 3 feet to the ground and landed on the piece of rock. He had lacerations to his face and left arm that required stitches. His face had broken bones.
Employee was loading garbage on 1 section. While dragging trash to the track to load flats he felt a sharp pain in his lower back. Note: Employee began missing work due to this injury on 9-5-13
As employee was stepping to unload some bolting supplies, he hyper-extended his right knee & pulled his hamstring - feeling discomfort & pain to hamstring. Doctor issued restrictions in which we are able to work with.
IE stated he was tramming coal hauler from the face back to the feeder and he ran over a crib tie and bounced up into the canopy injuring his neck. Due to several doctors and tests not finding anything wrong, we have waited until a IME was performed. The doctor from the IME verified his problems could have come from the accident.
Employee was unplugging a cord and sprained his wrist.
While riding in a maintenance vehicle, the unit hit a uneven undercut on the mine floor jarring the pick-up truck. The jarring action caused employee to have a pain in the lower back.
Employee alleges that she slipped in the women's locker room and injured her left shoulder. She was evaluated by an ER physician and taken off of work for a left shoulder fracture.
The employee alleges feel pain in the left arm and back.
Injured was hooking up flinger duster to scoop to take off hydraulics. He had position himself between scoop and flinger duster. He got himself between the scoop and fell on belt structure laying on the mine floor injuring his left knee. PPE was not a factor, mining equipment/system was not a factor, compliance of rules/regs was a factor, miner proficiency was a factor.
Employee was working on longwall recovery. He was in the shield legs and a 3/8 inch hose blew in the middle of the hose striking him in the arm causing a puncture.
Employee slipped on some wet material under the conveyor in the pit in July 2013. He felt fine after the incident and did not report it to the company. The employee started to have some discomfort with his left knee in early August 2013 he was place on work modifications after an MRI on 8/12/2013. with a sprained ACL/PCL Physical Therapy for a treatment plan.
EE was shoveling coal onto the belt line. When he felt pain in his back and right hip. EE went off work on 8/13/13.
Employee was pulling on a cat head and cable when he suddenly started complaining of back pain.
EE was welding and a piece of hot slag rolled into his boot, EE had his pant leg tucked in. EE worked the rest of his shift and part of the next before it started to swell. EE went back to his house and later to the hospital. EE had an infection set in and his Dr's. waited until the swelling went down before releasing him on 8/16/13 and back to full duty 8/20/13.
The employee was removing a piece of angle iron from a conveyor belt take-up when the angle iron fell and struck the employee on the left hand. The employee suffered a fracture to the middle metacarpal bone on his left hand.
Employee stated that while he was moving top belt cradles he caught his forefinger of his left hand between another cradle, causing him to mash his finger. He stated that he was wearing his gloves at the time of the incident.
Employee was carrying supplies underground, he slipped and fell striking his left arm. He was taken to the local hospital and x-rays revealed a fracture to his forearm. He began missing time on 8/12/2013.
Emp. was shrinking a bag over a pallet of bentonite. The plastic caught on fire. Emp. said he shed the fire off of the seed burner & was trying to put the fire out with the end of the weed burner when a piece of plastic lodged on his hand. Emp isn't sure how the plastic got on his hand. He may have swatted at the plastic with it. He doesn't remember
Equipment operator was crossing roadway to the readyline when he was struck by a man van. He was taken to the ER, kept overnight for observation, released with restrictions. Currently ONGOING.
Employee inadvertently put hand into a hot pile of ash causing burns to left forearm, left wrist and pointer and middle finger on left hand.
While stepping up into platform of a manlift aerial, stepped on piece of cut off wood and twisted ankle
Employee was pulling roof straps off the machine and stepping down from the roof bolter when he felt pain in the groin area. Employee was diagnosed with a hernia and had surgery to repair.
EE was changing the bucket teeth on a 992 Loader while using a hammer to knock the teeth off a piece of metal flew from the tooth and hit him in the left side of his stomach.
Fixing pump and was climbing out of pit. Co-EE reached to give him a helping hand up and IEE slipped as co-EE was pulling him up. Heard something pop in his back. Right leg went numb, left leg and lower back had a lot of pain.
Employee was lifting a timber and strained his neck. After further review of workers compensation records this incident has been reconsidered. Upon further consideration this incident/injury having been deemed compensable, this incident is being reported as a lost time injury.
Employee was installing a break away pin on the 1D Longwall when the relay bar pressurized and caught the employees right thumb between the pin and the pan line.
Employee was installing a belt on the mine fan and caught his right index finger and middle finger between the pulley and belt and fractured his right index finger and bruised his right middle finger. No Lost time.
Employee picked up a 40lb bag of rock dust to put in duster. While picking up bag employee felt sharp pain in lower back. Employee did not seek medical treatment until August 12, 2013 where he has undergone and will continue physical therapy for two weeks.
Haul Truck driver hit a rough spot on the haul road and was jarred. He was seen by a first responder, then taken to the ER. Diagnosed with back strain and given restrictions for 5 days. Restrictions couldn't be accommodated, and employee off work. Currently ONGOING.
Employee was roof bolting when he felt pain shoot in his back. At the time of the incident employee didn't take any medical treatment. Employee is now having pain and is seeking medical treatment.
Employee states that he twisted his ankle after stepping into a rut in the roadway. Employee was seen at CAMC General and referred to Sprains & Strains Clinic. Employee was placed off of work on 8/14/2013. No rut could be found at the alleged incident site.
Employee states that while working on the continuous miner a piece of draw rock fell and struck him on the (L) shoulder. Employee finished his shift. On 8-14-2013 he went to the Dr. who took him off work for testing and P.T..
Operator was on tractor cutting grass. He unknowingly hit a stump, the tractor came to a stop and he hit his head, resulting in a small cut on his forehead.
The employee was trouble shooting the battery bell from the scoop jumper cables while they were energized resulting in an arc flash that resulted in a second degree burn on the employee's left hand.
Employee was working on a ram car when he was getting off the mine floor he felt pain in his right shoulder.

Employee was carrying a bundle of pin plates in each hand to the roof bolter and tripped over a cable bolt and landed on his right lower back. On 8/7/13 employee reported an injury that the operator deemed questionable. On 10/17/13 the employee was referred to a specialist that reported the employee did in fact have an injury and that may have been treated incorrectly.
Employee was back out from under a truck. He stepped on a rock & twisted his right ankle. Cause of accident was lack of attention. Miner proficiency may have been a factor. PPE, company rules, mining equipment was not a factor.
Employee complained with back pain after normal work duties. A MRI was performed which indicated degenerative disc damage. A previous MRI from 2005 showed many similarities. Employee did not lose time until 09/09/2013 due to doctor's exam.

Knee was getting tired & so he stretched his leg out of operator compartment. The bucket was in the air & when it went down it broke his leg.
While conducting retreat mining on 6-Left Panel off B-4 Mains, a coal/rock outburst occurred inby break #35. The outburst involved the Mobile bridge operator who suffered multiple blunt trauma injuries involving the chest, right arm, and back.
Employee was getting in scoop and twisted his right knee.
Employees were loading rail onto a flat car. A rail was bound up against another rail. The employee tried to pry the bound up rail over the other rail. As he did this it sprung back and hit him knocking him down and then falling on top of his leg.

She pulled over to call technician to fix her computer, got out to clean windows and felt pain in lower back and neck. Taken to NNRH where she was put off work. Upon follow up, left off work to attend physical therapy.

The employee was assisting with screening the longwall face. The injured needed to remove a hook from the roll of screen. The employee assumed the shield was stopped and stuck his arm in the shield and got it caught on the shield.

Employee had just clocked out for the day and was walking down the wooden steps when one of the boards broke and sent him falling backwards and injuring his back. He went home that night to rest and came into work the next morning with his back still hurting. He went half way through his shift then went to the doctor.
EE lost balance stepping over 14"" pipe and prevented fall with forward step of L foot. Placement of foot resulted in fracture of #5 metatarsal L foot. Temporary restricted mobility.
Employee was retrieving truck sheet from loader. As he climbed up the ladder he felt pain in his right shoulder. He continued to work up until he had shoulder surgery on 3/20/2014.
Employee moved the RBM into the 3 left crosscut and dismounted the operator's deck. As he was exiting the deck of the RBM, the continuous miner trammed into the flypads directly behind the roof bolter, striking the employee and knocking him to the mine floor. This resulted in multiple sprains/strains of the upper body.
Employee was removing a slide skid from a flat car that was hooked to the motor. When removed the flat car moved backwards from the slack in the draw head causing it to flip downward which smashed Employees index finger on his right hand.
Employee was moving a spool of wire rope and felt pain in right shoulder. On 10/16/2013 employee had surgery to repair a rotator cuff tear in right shoulder.
While breaking stone, EE was hit in the left forearm by a foreign object, causing a laceration. No stitches were required nor time lost. However, on 9/30/13 a small metal shard was discovered at the healed wound site and removed. EE returned to work 10/14/13 at regular job at full capacity.
Employee stated that he was pulling back on a roof bolt to line it up with the drill pot. As he applied pressure on the bolt he felt something pull in his right shoulder. This resulted in a right shoulder strain.
EE lifted another ee in the loader bucket to replace a cylinder in the backhoe. Unbeknownst to them, the bucket was hung up on the arm of the backhoe. The bucket slipped and ee was pinned momentarily between the bucket and the backhoe. He was transported to the hospital and was kept overnight for observation.
Loading holes, moving Gravel, back became stiff.
Employee was helping to remove dozer blade, He was using a 4' pry bar to drive out tilt cylinder pin. Pin broke free causing the blade to shift a little and the pry bar hit employee in jaw. It broke his jaw tooth.
Employee was near the dragline bucket as repairs was being made. Another employee was driving a bucket tooth wedge in with a sledge hammer when a piece of the wedge being struck became a projectile and struck employee in his right eye. Employee was wearing his safety glasses at the time of the incident.
An electrician was going in the tub of a 2570 dragline to check the trial cable and strained his right Achilles tendon. On June 30, 2014 he was put in a soft case boot.
As the EE was cutting through pipe the portable band saw slipped and lacerated his R hand.
Employee was dragging rockdust hose into a drive area. He slipped and fell in a muddy area on his shoulder. Employee began losing time on 8/21/13.

Haul truck traveled down decline the employee engaged the retarded to slow the truck. The wheels of the truck skidded on the ramp due to the fact it had just rained which ultimately resulted in the truck overturned in the middle of the ramp. Employee was sent for medical evaluation and determined to have injured their lumbar.
The employee was building crib when a rock fell from the top, striking him in his head and sliding off his upper back. He began missing work because of the incident on 8/8/2013.

Employee was operating a scraper when he got stuck, the rig started bouncing and hit his head in the cab. He received a cut to the top of his head.
Employee was unloading trailer with lime and the agitator line came off end of hose and hit employee in the face-head.
Employees states that he bent over and picked up the end of a 6 inch Aquamine pipe and then turned to walk and felt pain in his back. Employee did not report incident to supervisor. He reported the event on 8/2/2013 @ 8:21 PM on the call in line. On 8/6/2023 employee called and asked if he needed to come in and have an incident report filled out.
Individual was attempting to crank down the manual brake on T-Motor when motor was moving down an incline in the haulage. Individual caught the left index finger between the brake crank and the frame of the motor causing a cut to the index finger. There were no sutures or fracture to the finger.
Employee was attempting to remove a water line coupling by inserting the sharp end of a pick hammer into a crack in the coupling and then striking the hammer with a claw hammer. While striking the pick hammer, a piece of the hammer head chipped off and imbedded into the side of the knuckle of his left index finger. The chip of metal had to be removed by a surgeon.
Employee stated that he was pulling a water hose through a crosscut to wash off the duster batteries. As he walked through the crosscut he tripped over a rock dust hose and fell to the mine floor injuring his shoulder. Medical received on 08/08/2013 diagnosed the employee with a left shoulder sprain.
The employee was in the process of opening a mandoor and as he was pulling the door open he strained his lower back.
Employee was pulling cable through conduit during a system repair and the bending, pulling, and stretching caused a lower back strain.
A chute was lifted by overhead crane with wire chokers & clevis's, chute was lowered back to the bottom to reattach chokers to adjust angle. The employee was inside the chute passing the chokers across, the chute slid down the slope appro. 16 ' stopping at the slope bottom. The employee attempted to move with the chute but it came in contact with his right foot and leg.
Employee was cutting a wiper for the skirt board on the crusher. The blade on the utility knife broke off and lacerated hand.
Injured was holding wrench and was removing a fitting on a hose when wrench slipped striking his index finger on Lt hand causing cut to finger.
Employee was assisting in the recovery of high voltage cable and got his right foot caught between the bumper of a ram car and the mine floor. He received a contusion to the top of his right foot.
While fabricating & installing a chute, employee bumped his knee on the adjustment bolt. He did not feel this to be a significant incident at the time. However, in the evening of 8-13-13 he sought medical treatment at an urgent care center and was diagnosed with cellulitis.
In attempt to remove a 50 lb bag from the production line, the bag tilted over forcing employee's right hand and wrist to be strained.

Individual was walking over a pile of coal behind the continuous miner when he hyper extended his left leg. Individual has had surgery and degenerative muscle in this knee in past months. He was diagnosed on 8/26/2013 that there was a new tear in the left knee and it would be a new comp claim.
Employee stated that while walking toward the guard house, she tripped on a drainage grate and fell to the ground.
Employee was shoveling and turned with the shovel full to walk to the belt, when he did his foot was stuck in muck and that is when he felt pain in his back. EMPLOYEE DID NOT START LOSING TIME UNTIL 9/10/13 DUE TO INCREASED PAIN.
Employee received a small cut under his left arm as he was helping put up a longwall cable bretby that had fallen. Employee did not start missing time until 8-6-13 due to infection.
Driver injured when seat of loader broke, pushing her forward in the cab and held her in an awkward position. The welds holding the seat had broken in two spots, allowing the scissor mechanism to force it forward. The driver requested no medical help or treatment at the time of the injury.

Employee was walking up a ladder when his retractable locked up causing him to lose his footing. He fell backwards off of the ladder and fractured both legs.

This office was just notified that employee was placed on modified duty on 9/11/13 per orthopedic consult. There is no lost time associated with this injury.
The employee was operating a twin head roof bolter. He put his drill steel in the chuck and swung the boom. The steel caught on a hog panel, snapped back and hit the employee in the face causing a laceration.
The Employee stated that he was scooping an entry and caught a roof strap, the strap came around and hit the employee in the ear cutting his right ear.

Injured employee was struck by a hoist cylinder when it unexpectedly broke loose from lifting devices, as it was being removed from a 777D rock truck. The 630 lb. cylinder struck his left leg as he was kneeling in close proximity to the work area fracturing the femur and fibula in his left leg.

2 employees were changing a top roller in the tail piece in 22nd West. While attempting to install the new roller, the roller slipped and smashed injured miners right hand between the roller and the frame structure of the tailpiece, resulting in a fractured Scaphoid bone in the right wrist.
Cleaning out rolls on crusher, standing on belting slipped metal edge hit right ankle rolled ankle cracked R. foot.
Employee was on level 2 standing between the roll up door and the man door, there is a step down to the man door. Co-worker called her name, when she turned to answer her she took a step forgetting about the step down and lost her balance falling towards the man door and into the guard rail. Here left side contacted the second rail on impact resulting in a fractured rib.
Employee was moving a copper starter sheet weighing approximately 12 Lbs. from a moving vertical rack to a horizontal conveyance system and felt pain in the right mid back area. Strain resulted in lost time of one day.
Individual was bolting on the right side of the Alpine Miner. He unknowingly activated the head moving it towards the mine roof (left hand on controls and right hand on steel). This caused the drill steel to bend with the right hand between the steel and the mast. Individual received 19 sutures to the right ring finger.
Installing a huggy clamp to couple 2 pieces of SDR9 8"" DRISCO pipe one end of the pipe released, striking the miner on the left cheek, temple and eye. Initial eval; concussion, released for 7 days of modified duty. Returned to work, 2 days later sent back for re-eval, admitted for 2 days, observation only. Released, diagnosed w/ post-concussion NO Work until follow up in 7 days.
Employee was removing drill steel from the drill pot when he felt a pain in his left shoulder.
Employee was working in the scale house and walked out to the safe area and tripped on some rocks and fell. EE refused medical attention at the time of the incident. About three hours later EE asked to go into RediMed and have her hand check out. RediMed stated EE had fractured her Left wrist and it was going to be splinted. At this time she was able to return to work.
EE was kneeling down discussing the completion of a job with his co-worker. When he stood up he experienced dizziness. He attempted to grab onto the handrail but missed. He fell face first onto the grating. He lacerated his mouth and eye and injured his wrist.
The technician was assisting in reeving a new rope through the retract horn on top of the shovel bucket when the rope moved unexpectedly causing the technician to lose his balance and fall onto the ground causing a hip and hand fracture.
Electrician was pulling wiring through conduit when he felt a hot pain in his abdomen
On 8/17/2013 this employee jumped off a roof bolter and twisted his left knee. He continued to work with no restriction until 10/24 2013 at which time he had surgery on his knee.

The technician was assisting in moving a 2800 P&H shovel when the machine lost propel capabilities. The machine then rolled in reverse striking the pickup driven by the technician causing the driver to be pinned in the cab.
Employee was retrieving nails from roof bolter when a piece of rib material, 6'L x 3'W x 12"" thick, fell, striking him from behind, injuring his left knee.
EE was descending a spiral stairway when he lost his footing and fell. EE's right scapula struck a stairway step resulting in a scapula fracture.
Employee was walking carrying a box of bag house bags when his back tightened up. Determined to be work related on 10/1/2013
Rocks were stuck between the top of the frame of the bed and the endgate. Employee tried to raise the endgate up with both hands. All of the sudden, the endgate swung closed. His right ring finger got pinched between the endgate and the frame.
Increased safety awareness training EE was operating front end loader and while backing loader rear wheel went over edge of inspection trench and loader rolled over.
Plant Operator was moving base stacker conveyor out of way for the Loader Operator. During this maneuver hydraulic hose broke. While searching for leak, either in hose or connections, he asked that it be raised up and pump turned on. In doing so pressure was reinstated in hose. Hand was positioned right over hole causing penetration to middle finger on right hand.
He bent over and when he stood back up he struck his hardhat on a pipe. *We dispute the allegation in citation #8055902 and submit this report solely to abate the citation.

We were changing a couple of links in the tracks of our 330 Cat track hoe and was driving the pins back through the new links when a piece of the Steel pin chipped off and Hit the employee in the thigh causing a small puncture wound.

Working on the counter weight belt B4. In process of removing an old guard, employee knocked a piece of steel loose. After being informed that the piece was loose, an approximately 20 lb piece of angle iron came down and hit his hand.
EE was swapping a material flow gate using a come-along and had to apply a lot of pressure. when the gate broke free, the EE's arm came back towards his body. Injury became reportable when EE was taken to the doctor on 9/30/13 and X-Ray revealed chip on elbow.
Employee was replacing a bearing on the head pulley on C-1. He was chipping on the shaft to slide the bearing on. As he was chipping a piece of metal hit his eye. This caused a large cut on his left eye. He needed two medical procedures to repair the eye.
Two employees were moving top roller assemblies up against the rib. They had moved several without incident. While moving the roller, the employee felt a pop in his lower back resulting in a back strain injury.
Driller was helping to place steel siding on newly constructed electrical building. Using a hand-held grinder equipped with a cut-off wheel. The driller was cutting a 36 inch wide piece of steel siding to dimension when the blade contacted his left index finger. Laceration between the first and second knuckle. Sutures and tendon repair required.
Ee getting into back seats of van. Started to lose balance, and braced his right leg to avoid falling. He felt pain. He reported to his supervisor on 08/07 and when the pain did not get better he asked to see a doctor. He was seen on 09/06 and referred to an orthopedist. On 09/12 he was diagnosed with a fracture to the tibial plateau and placed on restricted duty.
Employee was changing the tire on a John Deere skid loader. While trying to stand the tire upright the employee felt a pain in the groin area. Employee continued to work until 09/03/2013 when he had surgery for the hernia.
EE was attempting to remove a piece of stone from a turntable located on the hydrasplit. Stone was elevated about 8-10"" on the turntable. Stone slid off turntable smashing the EE's right index finger between stone and metal rollers on hydra split. The stone weighed approx.75-80 lbs.
An employee cut his middle left finger on the blower fan in the ceiling vent of a 992 D loader when the vent fell off, requiring stitches.
EE was swinging a hammer at a return roller on a belt conveyor and missed it. This caused his hand to in between the roller and conveyor belt.
Removing a pin on the barrel end of a Tilt Cylinder of a 992G loader and didn't have the linkage secured. When the pin came out the cylinder slid forward and the bar that was being used to push the pin out struck the Right hand pinky finger. Broken Finger
The employee was standing on the grating on the top floor of the expansion crusher near the drive of BC-59 when he pivoted to walk away and felt his right ankle pop several times.
Employee was removing a seal from a brake head. His hands slipped off while pulling on the seal, cutting both left and right thumbs.
Miner was surveying the pit when he stepped on a rock spraining his ankle, Miner was initially diagnosed with an ankle sprain but returned to the doctor two weeks later and was re diagnosed with a foot fracture.
Employee was assisting in the placement of a bundle of rods. The rods slowly swung toward the employee and he put his right hand up to steady them. The rods inside of the bundle shifted catching the tips of his middle and ring fingers of his right hand. Per employee, butterfly closures were used.
Employee was holding a bar to hold a wheel in place working on the dredge. Wheel rolled out of position and onto his fingers.
Injury occurred on Thur. 08/29/2013. It was not reported until Tue. 09/03/2013, at which time employee was sent to the doctor. On 08/29 employee was swinging the large hammer to break a large stone. When he hit the stone some rock fragments flew off and hit his left hand. He said he was wearing only one glove.
EE was working on the north line installing saddles on the bleeders, came on a saddle that was plugged. Was using a small pry bar to unplug saddle drain discharge line as he was cleaning the saddle the EE's right pinky finger came into contact with the discharge line nipple and small pry bar. This caused a laceration and fracture to EE's pinky finger.
Lifting a wooden bridge plank when he felt something pull in upper back of right leg. (torn ligament)
Miner was positioning a 6""X10' section of drisco pipe into the pipe welder clamp. Employee did not move his hand out of the way when the pipe was pushed forward by other employees causing his right middle finger to become caught between the clamp and the pipe.

Employee state he was loosening air line fittings from pipe fittings with two wrenches. Upon squeezing he wrenches to loosen the fittings he heard a tearing sound and burn in his left shoulder. Strength was weakened in left arm, thought it would be better. By Friday evening, bruise had formed on left bicep and pain persisted. DID NOT REPORT INCIDENT UNTIL 9/2/13 late evening.
Employee was moving pallets and was punctured by a rusty nail in his hand.
As an employee was working on the S Dedicated pad, a piece of 8 inch pipe contacted her in the leg causing a laceration. The employee was transported to Nye Regional Medical Center where she received 1 suture to close the laceration.

A mechanic from the Mobile Shop received a laceration in his left hand. He was changing a seal on a PTO. The tool that he used slipped off his hand. He received the cut on the palm of his hand close to his thumb.
Employee was handling a chain clevis when his left index finger was caught in the clevis tearing skin from fingertip.
Drilling face with Jackleg, went to move machine, the leg was pressurized causing the leg to extend thus pulling right shoulder out.
Employee was shoveling material from catwalk on the screen plant resulting in strain to triceps and deltoid, employee is on restrictions.
EE was unscrewing a bale, the pipe turned in the clamp, the bale fell on his hand, crushing the hand between the clamp and bale.
Employee strained right shoulder while carrying a piece of pipe. Employee did not initially seek medical treatment. When pain persisted, employee saw a physician on 9/6/2013 and was placed on restricted duty.
Employee tried to drive a broom under a low clearance bar, causing the roof of the broom to break loose and strike the employee on the left index finger, and on the head. The left index finger required two stitches.
Employee stepped on uneven ground and rolled his ankle.
Employee was attempting to open a valve for a back up slurry pump, but was unable to get it to open and restarted the primary pump. When pump started an over head slurry line ruptured and sprayed slurry on him blocking his escape route. The employee climbed over the hand rail and jumped about eight feet to the ground causing a fracture/ dislocated right shoulder.
Employee stated they strained their back while installing tire chains. Employee did not seek medical treatment until 08/29/13 when he was placed on modified duty.
A Section Supervisor hanging up sight rods for center lines and a piece of draw rock fell from between the bolts and struck the individual in the back. He was diagnosed with severe degenerative spine disease. Note: This accident was originally reported on 8-30-2013 to MSHA data base and for whatever reason was not retained.
Operator was disconnecting discharge hose at #4 acid station and hose blew off and sprayed him on side of face and back of neck causing employee to fall and sprain his wrist.
Using overhead crane with cable employee placed the cable on one of the lower bolt & nut on press head. This was in an effort to help turn the press head for opening. Using the remote control in right hand for the crane, he inadvertently tightened the hoist cable jamming is left hand between cable and press head. Severely bruised left hand. He was placed on restrictive duty.
IE was tramming miner, he was walking in a right to left direction. The miner was stopped and IE stepped into some uneven bottom and felt a popping sensation in his lower back.
During rounds at the crusher at the 1026 belt, the employee's ankle rolled while walking on the cat walk. There was nothing in the walkway and no other hazards present. Taken to company Dr on 8/26/13 and placed on restrictions.
Two employees were removing a portable eyewash station from a wall when one of the employees felt a pop in his elbow as they were setting it down. He was taken to the clinic, referred for an MRI. Results of the MRI, employee given lifting restrictions on 9/12/13, and referred to 8 physical therapy sessions.
Employee was walking down the ramp when she slipped on loose gravel causing her twist her knee.
The truck shop boiler makers were assigned to clean up behind the bed repair area. Employee was directing using hand signals for EE on the forklift to separate two sheets of 600 plate. Employee signaled the operator to lift the plate to install a 4x4x4 piece of wood to place between the plates. While twisting and pushing Employee felt a pull in the lower center of his back.
He was in the pit, climbing off his bulldozer. There was a trench that he stepped on and he twisted his L ankle on the uneven surface. Ankle is swollen and doesn't move normally.
Employee twisted his upper body while holding a grate and felt pain and stiffness in his right knee. Employee was diagnosed with a knee strain.
Employee was bolting up a round that had been previously blasted. He was in the process of tightening a dywidag bolt when a rock directly over his head fell and contacted the front of his hard hat. This caused what was diagnosed as a neck sprain.
The employee was picking up a Jackleg Drill off of the ground and strained his lower back.
Employee was tightening a large bolt with a ratchet. The ratchet slipped and employee's right hand struck solid steel structure smashing his right pinky finger between the structure and the ratchet. Employee's right (pinky) 5th finger was crushed. Employee was restricted.
The injured miner was changing out oxygen cylinders and when he set down the rack one cylinder in the rack shifted causing his finger to be caught between the cylinder and the rack.
He was using a pry bar to change a track rail when he lost control of the bar and struck himself on the top of his handset. *We dispute the allegations in citation #8055896 and submit this report solely to abate the citation.

A Truck Driver suffered a sprain to his right shoulder while climbing up into the cab of the Water Truck. The miner states his right foot slipped off the bottom step and to prevent from falling he held on with his right arm causing the sprain to his shoulder.
EE was cleaning the built up material on the burner pipe for the kiln when he said he felt a sharp pain in his left shoulder. EE did not report the injury until 8/30/13 and refused a medical evaluation. EE requested to be seen by Dr on 9/19/13 and EE was placed on modified duty that day.
Lifting a screen in place on the top deck of screener
He was operating a front end loader dumping sand into the hopper. While driving up the ramp the loader ran over a large rock jolting the operator in his seat. Injuring his lower back and hip.
Employee claims that he stepped on a nail protruding from a piece of conveyer belting.
EE was helping set up a port. Conveyer, he was handling the power cable when the cable was moved, pinning his left wrist between the cable and the cable bracket. EE suffered a broken bone in his left wrist.
Employee was in the process of closing the discharge valve of #1 Spent Liquor Pump by hand when he felt a pop in his lower back. Strain.
Mine surveyor was walking across the shot muck performing ore control, stepped into a covered shot blast hole and twisted right ankle.
Employee was adjusting the outrigger on his mechanic's truck. Held on to outrigger unit by placing his finger in the lowest adjustment setting hole. His hand slipped and the outrigger foot fell trapping his finger between the outrigger and the outrigger housing.
Employee was lifting 8"" pipe with a 2"" pipe inside, when pipe was set down, the 2"" pipe bounced up and struck employees left middle finger.
Employee was slipping a cable choker, which was attached to the John Deere tractor, through the dump rope bucket eye when the choker tightened unexpectedly. This motion resulted in the pinching of the right index finger between the choker loop and the eye of the dump rope.

While working to rod out feed drain for 2-4 heater to cross block 2-3 heater for isolation, EE stepped onto scale and dirt buildup under flash tanks from cleanup efforts in the area, causing him to twist his right ankle. Ankle Strain

The Employee stated that he loaded a shuttle car, the shuttle car operator went to leave and over steered bumping him on his left side.
Employee was cleaning out the Pioneer shaker screen using a piece of metal. The piece of metal slipped and smashed his little finger on the left hand.
Employee was moving a jackleg drill and dropped it on his foot. He was seen at the ER, diagnosed with a contusion to the foot and released to restricted duty. Upon follow up on 08/28/13, he was released to full duty.
Coarse material was loaded into a Cat 772 by a Cat 988, which caused the truck and operator to be shocked very hard. The truck operator was leaning forward on the steering wheel, and his seat pressure had been lower for road comfort. The shifting truck caused him to be slammed into the seat which hurt his upper back and made it hard for him to breath (thoracic strain).
While working on airlock doors EE moved to let mantrip open doors. A piece of falling material fell striking him on back of neck causing him to fall to ground. This is being filed to terminate citation written during a Part 50 Audit. We do not agree this was reportable. We do not accommodate ""restricted work activity"". Time sheets show EE was back at regular job next day.
While placing a pipe onto a pump, the pipe rolled and pinched left middle finger between two flanges, broken finger
Prestrip operators were assisting shift maintenance in the installation of a roller on the 239 conveyor belt. Employees were working on each side of the belt. As employees on one side of the roller positioned it into place the other end of the roller moved and pinched employee's left ring finger between the roller and the mounting bracket.
While trouble shooting a possible short in a wire that was located in a cable tray EE inadvertly caused the wire which had became bare to contact the cable tray and caused a shock.
Utilities worker was rebuilding a flushing head for the drill jumbo when he hit his finger with a hammer
employee was putting Inbed structure together when he mashed the tip of his left thumb, A bone chip was found and was removed 8/16/13 and he received stitches.
Two employees were installing a metal liner that weighed approximately 100lbs. inside of the secondary crusher feed bin. One employee lost his footing and dropped the end of the liner, causing a jolt to the other employee that was still holding the liner, resulting in a sprain to the right arm.
