The hoist car and slope lost power due to failed transformer located at the #1 portal. The hoist was back in operation at 5:10 AM. No men or supplies were on the hoist car at the time.
A contractor who was installing piping on the top of the cement silos became trapped when the elevator became stuck for appox. 2.5 hours. An elevator service mechanic was called out to the plant to get him free.
The hauler operator was traveling toward the crusher along the main haul road with a load of lignite. The employee dozed off right before the DD7 Diversion causing the truck to go to the right, toward the concrete barricades. The truck traveled through the barricades and came to rest on its right side. The employee was removed from the cab utilizing a JLG lift basket.
A roof fall above the anchorage occurred on the mainline #14 Belt at 20 crosscut. The dimensions of the fall were 16 feet wide by 120 feet long by 15 feet high. No injuries or entrapments.
Roof fall occurred in North West seal line at seal #28 in the intersection. Approximately 20 ft wide and 30 feet long and 3 1/2 feet above anchorage. No one was hurt or injured. Airway or travel way not blocked.
An unexplained roof fall (slip) occurred. No equipment, entrapment, or Airways were interrupted. The Slip was 5' wide X 4.5' thick and 15' long. The location of the slip was at break 217 on #12 belt. This fall was not above anchorage, but the current MSHA inspector felt we should report it.
At approx. 3:00 am, an employee became trapped on the elevator due to extreme cold temperatures affecting the over travel switch. The employee was able to get off at 4:18 am with no injuries.
An unintentional roof fall in inactive works occurred on the 2nd mains, at 6c/c, between #8 & #9 entry at spad #813. The fall was 20'W X 60'L X 6-8'H. No persons were injured and ventilation was not disrupted. The area will not be recovered. The area was breakered off and barriers erected to prevent entry into the area.
At approx. 6:30 am 11 employees became trapped on the elevator due to extreme cold temperatures affecting the over travel switch. The employees were able to get off at 7:11 am with no injuries.
An electrical contactor located in the hoist house motor control panel became stuck causing the hoist to not function properly. The hoist was restored to service and tested. Miners returned underground at 10:12 p.m.
A non-injury roof fall was discovered at SS#3950 on the 2-West Mains by EE at 5:30am.
Investigation of the primary escape way air shaft revealed that large amounts of ice removed from the shaft liner has fallen and broke the left guide rope eye bolt anchor provided for the emergency escape hoist at the bottom of the air shaft.
Automated control system shut down the coal mill as designed after detecting a high temp at the mill outlet. An error in the programming logic associated with the inlet air damper caused the damper to not fully open, stopping cool air from entering the mill. During the restart of the mill a loud noise was heard within it. There was no damage to the mill and there were no injuries.
A contractor was working on demolishing a crusher stak, that has been decommissioned by Lehigh SW Cement. The contract employees were cutting a conduit, when slag material fell into the apron of the jaw crusher, and caught residual grease, that was not visible from their working location on fire.
AN UNPLANNED FALL OF ROOF HAS OCCURRED 1 BRK INBY SPAD NO.7027 IN RIGHT RETURN OF NO. 6 WEST MAINS. THE FALL MEASURES 100 FEET LONG, 20 FEET WIDE, AND 10 FEET THICK. ALL ENTRANCES TO THE FALL HAVE BEEN CRIBBED AND DANGERED OFF. THE FALL DOES NOT AFFECT VENTILATION OR IMPEDE TRAVEL IN THIS AREA.
An unintentional roof fall has occurred on the Left Return, 4th Left Panel #7 Entry Moles Sub Mains 1 break inby spad 9904. The fall was approximately 20ft x 26ft x 5 1/2ft. There were no injuries reported or no equipment was involved.
Gourmand wheel counter weight would not release from the bottom of the #9 elevator. No men were on the elevator at the time of the incident.
The Track limit cable broke at 8:30 pm on the hoist at the Pend Oreille Mine no one was affected as all employees was above the 1085 level. the cable was fixed by two mechanics and the hoist was back running at 10:20 pm
workers in the area found the fall. The fall started in the belt entry #5 entry and fell approximately 12 feet past #6 entry. The fall is approximately 18 feet wide and 70 feet long it fell on approximately 22 feet of belt structure. The fall was in a isolated area of the mine where adverse roof conditions are encountered. The fall area is at spad number 59+60.
A fire was discovered at the hourly bath-house trailers, smoke was seen coming out of the roof vents by a worker. Fire was investigated and found in the ceiling area, fire dept. was called and fire was put out at approx. 4:00 pm. Trailer is a total loss and no injuries were involved. Cause, electrical short in the wiring. Compliance/rules, PPE, Mining equip/systems not a factor.
A.C. side of generator shorted out on the brush holder on start up.
Labor Finder Laborer ws riding the K3 elevator and it stopped between the first and second floor. Executive Elevator Co. was called out to troubleshoot and reset the elevator. The employee exited the elevator at approximately 3:27 PM. There were no injuries as a result of this incident.
Dozer track came in contact with a large rock near the surface causing dozer to slide sideways resulting in a roll over.
EE was taking the passenger elevator in the packhouse to the top of the cement silos when the passenger elevator stopped in between the third and fourth floor. Operators were sent to check the elevator doors and all were closed. An elevator tech from Thyssen Krup was called out to get the trapped miner out. He was out of the elevator at 4:20 am.
The person responsible for the daily inspection of the elevator got stuck on the elevator near the bottom. Schindler Elevator Service was called to evaluate and correct the issue. Ice build up on a door switch was found to have caused the issue. The employee was freed at approximately 5:45 AM. The elevator was throughly examined, evaluated and returned to service.
The crabapple elevator stopped part of the way down in the shaft with employee on Board. EE was on the elevator for 55 min. He was not injured as a result of this incident.
The slope hoist was removed from service due to the speed sensor malfunctioning. The sensor was repaired and the hoist was placed into service.
Increased seepage was noted near the toe and #3 slot drain of D cell, the D cell is under a k order and under the approved plan we were instructed to call MSHA if we needed to bring equipment on the west embankment to control the water runoff, etc. No injuries or evacuations have occurred as a result of this event, we are managing the water on site.
Brake switch would not release brakes on hoist car. Brakes were frozen.
Employee set fire to a pile of old tires. This is a reclaimed mine. No employees on site. No active mining on site.

The submission of this form 7000-1 does not constitute an admission by U.S. Silver and Gold that an entrapment or a violation of 30 CFR 50.10 occurred and no inference of either event may be drawn from its submission.
The Crabapple slope hoist was not operational for an extended period of time. Electrical issues in the power feed lines caused the interruption. No injuries as a result of this incident. This slope hoist is not used as any type of escapeway.
A roof fall above the anchorage was discovered on the mainline 13 belt at 37 crosscut. The dimensions were 20' long by 20' wide by 15' high. No injuries or entrapments. Ventilation not affected.
A roof fall occurred in panel 11 #4 entry Crosscut #23, this was in the secondary escape way. MSHA was notified by telephone on 1/18/2014 at 1:17 P.M. The lifeline was re-routed, miners were advised of the change and the escape way map was also revised. All intersections leading to the fall area has been barricaded off and supplemental floor to roof support installed.
NOTIFIED AT 12:15 AM THAT THE EAST FINLEY ELEVATOR WAS DOWN. MAINTENANCE WAS NOTIFIED AND REPAIRS STARTED. AT 1:30 AM ELEVATOR WAS BACK IN SERVICE. BOTTOM DOOR MOTOR WAS REPLACED.
On 1-18-14 around 9pm at an hr after our quarry closed and all shifts ended, a fire started and burned a piece of equip (arson?). The fire dept. was contacted and put out the fire immed. equip was completely destroyed and it burnt the conveyor belting next to it(minimal) The burnt equipment was discovered by an employee the next day no injuries and MSHA was called immed.
Unintentional roof fall inby spad #s 12185 and 12039.
A roof fall occurred in Panel 11 at crosscut #34 in the #2 entry. This roof fall was reported to MSHA by telephone on 1/17/2014.
A roof fall occurred on the 8 South mains in the #2 Entry at spad location 55+45.The fall was 16' wide, 10' high, and 18' long. A slip/clay vein in the intersection was a contributing condition. It didn't involve any compliance issues. No equipment involved. No other 50.20 criteria applies.
An unplanned roof fall has occurred in the No. 5 entry intake escapeway the fall measures 22'long, 24' wide and 48"" high the fall does not impede travel or affect ventilation. all entries around the fall has been cribbed and dangered off.
On 1/15/14 an inundation of mud occurred in Pit 27 on E highwall. This caused material to be deposited on Komatsu 2000 excavator to railings of opr's deck, around R side of excav. & in front of opr's compartment. Cat 992 loader on S side of slide had mud deposited on L side of wheels. No inj. as a result of slide. Pursuant to cit. issued # 8482945 this report is being filed.
On North A track, at 15 cross cut, in the adjacent entry, against the rib, a roof fall occurred. It measured 30 ft. in length and 9 foot in width. Around 12 inches above anchorage.
The contacts burnt up on the drive motor causing the elevator loose power. The 1-800 number was call multiple times from multiple location and there was no response. We also tried to contact another person and there was response from him either.
During a weekly escape way examination the third shift fire boss discovered a roof fall in the 1Rt panel off of 2 East section just inby spad #2561. This is an area that has been mined out and no longer in producing status.
A fire occurred in a compartment of the GCC Cooler as a result of a hydraulic line break, when the kiln flushed.
Top fell during second mining pulling roof bolts out by Mobil roof supports.
Contractor employee was entrapped in coarse crusher step 1&2 elevator at 2:51 p.m. Entrapped was ee from Lakehead Constructor. Entrapped for 45 minutes. US Steel employees 468, 27 contractors.
Electrical Fire in shed which houses all electrical boxes and panels that supply electricity to plant.
An unintentional Roof Fall occurred at break #6 of the # 33 Beltline. Fall dimensions are 35' long x 35' wide x 8.5' thick
Coil that pulls in the safety circuit reset cellunoid failed. Replacement was available and required 10 minutes to correct once an electrician was called to site.
Miner was cutting in the #3 entry near S/S 1090 on the 3-Right Section when it cut into a borehole. The borehole was on the mine map and was not fully grouted, causing an inundation of water to come into the section. The water did not impede ventilation or escapeways and was plugged immediately.
A power outage to the mine tripped a breaker on the koepe compressor for the koepe brakes. It took longer than expected to troubleshoot and find the issue. The breaker was reset and the koepe was put back into service on 1/12/14 on third shift. No Damage, No Injuries, No Issues.
Roof fall was discovered by the pre-shift examiner on the 3 NE Submain (10+90) xc#13, Entry #6. The fall was in the intersection approx 20' in length, 20' in width, 10' in height. There was HV cable under the fall.
The fines crusher, step 3 crusher, lost power entrapping an employee. 89 US Steel Employees, 4 Contractors working on site. Entrapped miner was Supervisor, 22 weeks miner experience.
An unplanned roof fall occurred on the Main West, Crosscut 25, Entry 4. The fall measured approximately 20 feet wide, 20 feet long, and 10 feet high. Compliance with rules and regulations was not a factor. The fall will be cleaned up in compliance with the mines roof fall cleanup plan.
A CO ALARM WAS NOTICED AT THE 2 EAST MAINS TAIL PIECE AND THE SECTION FOREMAN WAS NOTIFIED. HE FOUND FLAMES COMING FROM AN INTERNAL BEARING ON THE SECTION TAIL ROLLER. HE USED A 1/2"" HOSE AND EXTINGUISHED THE FLAME IN LESS THAN 3 MINUTES. THE FIRE NEVER LEFT THE BEARING HOUSING. THE HIGHEST CO READING WAS 12.5 PPM.
Ice fell down the intake (koepe) shaft and struck the H beam causing damage. The H-beam allows the balance rope to rub so that it does not tangle up. Employees worked to repair the H-beam and the koepe was back in service on 1/11/2014 on Second Shift. There were no injuries or other issues.
Hoist down due to cold temperatures, sensor iced over.
An unplanned roof fall occurred on the SW Mains, #5 Entry, #10 blk, spad #1652. There were no injuries or equipment damaged due to the fall and no escape ways or air courses affected.
A reportable roof fall was found in E East Mains, entry 3, crosscut 6. The roof fall was approximately 35' long, 21' wide, and 9.5' high.
While doing field service on hoist a defect was found on the wire rope and had to be retired from service. A new wire rope has been ordered, the miners have been notified and the refuge chamber is operational.
An unintentional roof fall has occurred Outby #2 Section 50ft inby spad 3977. The roof fall was approximately 60ft x 16ft x 5ft. There were no injuries and no equipment was involved.
The elevator car stopped 85 feet from the top landing. The car was moved to the top landing within 15 minutes. The actuator for the slack rope switch was bent.The actuator was straightened, sheaves and ropes tested, and inspected the car. the elevator was returned to service at 12:15pm
Brake linkage arm on the main hoist wore at a pin & bushing connection causing the mechanical linkage to bind and then crack and break the end of the arm.
At 12:40pm the electricity providing power to the main slope hoist was disrupted. Fuse on incoming line was found in disrepair and replaced. Hoist was operational 1:20pm.
At approximately 2:00 p.m. there was a rib roll discovered at brk. 31(SPAD #1460) in the North Mains intake escapeway entry. The rib roll knocked three cribs which were previously built and blocked the escapeway. The men were withdrew from the mine until escapway was restored.
At 3:10 am two EE's were trapped on the elevator when in went down due to cold weather.The car safety switch and counter weight buffer were adjusted and the elevator was repaired at 4:54 am. The EE's were able to get off at that time. There were no injuries.
A Fault in the brake electrical system on the elevator.
The cage was returning to the surface empty. Approximately 50 ft off the bottom the cage stopped. Initially this was thought to be caused by an open gate on the cage. Upon further investigation it was learned that the power/communication cable to the cage had fouled on something, pulling the cable in half. Notified mine management and appropriate regulatory calls were made.
Unplanned fire occurred at the Wyoming #2 mine in one of the office trailers. The fire was extinguished by the dispatcher within about 20 minutes of discovery. Brenton & Pineville fire dept were also dispatched to the seen. It is believed the fire started in the heater of the maintenance chiefs office. The trailer has been removed from property. no injuries were reported.
At approx. 12:10 am on 1/5/2014 a roof fall above the anchorage was found in the F (intake) entry of the 6Left off 4North section. The roof fall was at survey spad #21+10 and extended inby to about 21+50. The fall was about 40ft. long 17ft. wide and 6.5ft. high. All areas were dangered off around it and the escapeway was rerouted. There were no injuries or property damage.
The main bearing on the east side of production hoist began to fail. The hoist was shutdown, and the bearing replaced. Hoist was placed back in operation at 2:00 p.m. on 01/08/2014.

Pumper discovered roof fall above anchorage (13' high, 16' wide, for unknown length) on backside of small sump area by K-Seals #2 Seal. Neither ventilation nor travel was restricted. Area will not be cleaned up.
Schindler called for electrical fault on Sugar Run elevator. At 12:53 pm they discovered a retaining bolt for a disconnect switch had backed out (due to rust). Repairs made immediately, returned to service by 1:30 pm.
Elevator doors would not open due to switches not working properly. No men were on the elevator at the time.
An unplanned fall of the mine roof above the anchorage horizon occurred on the 7 Right section at 11 block, crosscut 2-3. The fall measured approximately 9' high and 86' long.
Was mining in #2 entry of E26 section 24 feet from center of 27 x-cut when ignition occurred. it was orange in color and lasted 3 seconds and was extinguished by the sprays. The miner was cutting at mid face just under a binder.
A roof fall was found at 67 Mains, on 6 track at break 8 between 4 & 5 entry. The fall was approx. 2' to 3' thick 18' wide and 30' long. 10 Heintzman Jacks with pizza pan heads will be set. This fall does not block any escape way or effects the ventilation.
The #8 elevator was out of service for a period of time exceeding 30 minutes due to a bottom door lock fault. The elevator was tested after repairs were made prior to returning to service at 8:05am.
At approximately 3:15am the Shindler Elevator became inoperable because the automatic doors would open. The processor was rebooted and the Shindler Elevator was back in operation.
Elevator was down due to an electrical problem.
The elevator at #2 Portal stopped with 1 man on board. The entrapment lasted 36 minutes. The elevator was found to have a broken lead in the call switch and 1 lead in a contactor. All were repaired.
An unplanned roof fall occurred in the #9 zone worked out area. The fall was discovered on 1/2/14 by the shift foreman. There were no injuries or equipment damage due to the fall and no airways were obstructed.
Employee was caught between the feeder and the coal rib as he was tightening up the chain connecting the feeder to the tailpiece. The crew had just completed moving the belt and was in the process of anchoring and aligning the tailpiece. The tailpiece broke loose and when doing so shifted the tailpiece and feeder towards the off side of the belt, where the employee was positioned.
Employee was pulling on a pump suction line in a frozen pond when he felt a strain in his stomach. he went to doctor and diagnosed with a very small umbilical hernia but employee did not treat. Employee has returned to his regular job with no lost time or restrictions.
Not recognizing icy conditions while performing shift inspection, causing slip hazard, which resulted in a fall that ended up with a loss of 1 1/2 fingers on right hand.
EE was helping change out 4"" water line. Pipe got stuck in mud and ee picked up on end of pipe to free it. 1 of other EE's was on other end of pipe using a bar to help move his end over. Water line shifted and caught ee left ring finger between the pipe and a timber, causing a fracture. Now had left ring finger amputated at knuckle
Employee was getting ready to operate plant. His right arm was caught between the bearing and tail pulley. Right arm amputated and face laceration.
Employee was pushing a hand tool cart in the shop area and felt pain in her lower abdomen
Using 25 ton hoist in Metz Motor barn to lift C motor by attaching to old slope chain bracket. EE was installing a bearing housing when slope chain bracket weld broke off motor frame, motor fell, partially amputating right thumb (between first and second joint) between the motor and bearing housing. Company block to final working height procedures were not followed.
Employee was lifting a 95# cement bag off a conveyor and felt a pain in his abdomen.
992G Loader picked up rear end of a 992D Loader to crib the machine. Loader man let bucket down and employee had placed his fingers in pinched spot and left finger and thumb were smashed.
While operating a scoop tractor, the employee went to exit the operator's compartment. While exiting, the employee's left hand slipped off the door framework. He felt a ""popping"" sensation in his left shoulder. The accident did not involve any aspect of compliance nor PPE. The employee was an experienced operator with current task training on this piece of equipment.
The EE was unloading a leaf spring at the end of the track. The EE lost his footing and fell into the manbus-pinching his right hand between the spring and the manbus,resulting in injury to two of his fingers.
Employee was loading acetylene bottle onto rubber tire personnel carrier when he felt a pain in his left shoulder. The result of the injury was a strained muscle in left shoulder.
Truck driver working for a hauling contractor was rolling up the tarp on his trailer when the handle kicked back striking him in the ribs.
When he was carrying an angular tripped falling back and began to feel pain in the back.
Re-establishing angle of repose on spoil bank. Bench collapsed and excavator fell to mine pit. Operator received broken ribs, fractures orbital socket and a laceration to right eye.
Trying to adjust track on excavator when a spring flew out hit him on the head.
Employee was walking from the scale house to his vehicle. He slipped and fell causing a fracture to the fibula bone on his left leg. The weather conditions was clear 28 degrees with the sun out.
This incident did not immediately involve either modified duty or lost time. For more than three years EE has had incidents on this job with his left ankle. Yesterday(05/06/2014) he came in with a notice from his doctor stating that he would be out of work for at least 30 days following surgery on his left ankle to correct the problem.
Employee slipped and fell inside the preparation plant.
Employee stepped on a rail tie and rolled his left ankle resulting in an avulsion fracture to his ankle.
He suffered a cut on the finger of the right hand. This happened while cutting chain with acetylene the D-11N. He put his hand on the chain that stay suspended in the final drive, but by cutting the link chain moved and pinched his finger.
Employee was helping install Hilfickers around the inlet and outlet of a fifteen foot culvert. Employee stepped over the four foot rise to retrieve a rake that was stored inside the culvert. Employee lost footing and slipped puncturing upper leg with a 3/8 inch wire located on top of the hilfickers requiring two to three stitches.
The miner received arc flash burn to his eyes. The miner was working with a welder in our welding shop. They were working back to back. The miner received the arc flash from the reflection off the inside of the welding helmet he was wearing.
The wheel of a loaded refuge chamber struck employee and pinned his ankle against the rib. He was diagnosed with a fracture to the right ankle.
EE was cleaning out a service truck, while emptying the trash he cut his hand on some glass
While walking on road bed material of 5/8"" crushed rock, employee stepped on right foot wrong way and rolled right ankle. Went to Doctor on 31 Jan 2014 found to have fractured bone in right foot.
Individual was loading Degas pipe with another employee when lifting and pushing the pipe onto a mine car the individual felt a pop and pain in the right shoulder and neck area.
EE was working on putting in a head drive. He was on a ladder, went to climb down when he got to second step his foot slipped and he fell and strained his left knee.
Injured employee stated he was swinging the twin boom roof bolter boom to install another bolt when loose material fell and struck him on the back of his head and neck.
EE was hauling dirt. While in the yard he noticed a large rock between two tires on his trailer. He grabbed his hammer and started hitting the rock to get it out of there. Hit it a couple of times and then both tires blew while he was hitting at the rock. After the tires blew, the loader operator looked and noticed him on the ground. He called his manager to call 911.
Employee was lapping belt on 43 break on #5 belt and a loose rib fell out onto his right leg.
Employee was bolting off a Joy 14-09 mining machine. He stepped over some gob and hyper extended his left knee.
Injured was walking into portal building, slipped on first step and fell to the ground landing on his left hip.
Individual was in the process of untangling the red wash down water hose from the miner cable and water line when he felt pain in the lower back area.
Employee was pulling a brow with a slate bar on the right inby corner of #3 Intersection of L3B. While doing so the brow fell and pulled slate bar out of his hands, both brow and slate bar fell on his left foot.
riding rubber tired mantrip from end of rail to section. went over a hump his head hit a roof bolt. cut his forehead.
The miner was pulling cable off the shuttle car reel so he could hang it. While pulling cable off he felt pain in his left shoulder. He was taken out of the mine to be looked at.
Employee was loading Hyd oil onto the 4 wheeler and twisted his knee.
Stepped on a rock while walking toward right side dinner hole and twisted left knee. Company rule of making work area safe was not followed. No mining equipment involved, no regulations broken, no issue with PPE and no issue with training.
He is a miner with limited experience running a Joy 10SC32 Shuttle Car. He failed to recognize the hole in his tram road due to an inadequate examination during his pre-operational check. He dropped his shuttle car into the hole in the tram road and the jarring of the tire dropping down caused him to strike his left knee against a metal bracket in the operator's deck.

Employee bent down to pick uo miner cable and felt pop in lower back.
Had some packed snow on the ground. EE did not notified anyone that he had slipped. He continued to work until 2/5/2014 when I received a call from the hsp that he had been admitted because he slipped and hit his head. I then filed a claim with our Workers Comp Carrier. We are still not sure if he fell at work, that is being investigated by the Workers comp Carrier
EE was lifting a heavy slushier bucket on an outside muck pile to turn it over. The muck under his feet shifter resulting in the strain - injury
EE was pulling on a 60 inch pipe wrench and strained groin area.
Employee was unloading timbers from a scoop bucket and 2 of them were frozen together and he strained his groin while trying to pull them out of the bucket. He has been diagnosed with an inguinal hernia which will require surgery. He started missing work on Feb. 12, 2014.
employee was knocking out a stopping, he was knocking out the top row when some roof material struck his extended right arm. he completed the shift and worked a full shift on 1/28/2014. He went for medical attention on 1/29/2014 and was given two days off for what was found to be a blood clot.
Employee received a laceration to the right thumb when the thumb was caught between a crescent wrench used to hold a nut and the frame of a separation screen. Torque applied to the bolt by a co-worker caused the nut to twist and pinch the thumb between the handle of the crescent wrench and separator frame.
Employee was moving part upstairs and felt twinge in lower back. Diagnosis is acute 4 S strain.
I was trying to find an oil leak on a D10T while climbing down front of left track my feet slipped on snow & ice on track & I fell straight down & landed on Rt leg hurting my lower back from the landing.
Employee was roof bolting on top of a cut overcast. Stepped on a stone and strained knee.
EE was installing last bolt in the face of #3 entry of #3 MMU. He put the top section of his finish steel in the roof and the bottom section in the chuck of the bolter. He coupled the two sections together. As he removed his hand , he hit the rotation and feed lever. The pressure from the boom caused the steel to bend and as it rotated, it struck EE in the arm.
Man trip struck a broken fish plate which caused the trip to come to a sudden stop. Other employees slid against him and he twisted his knee.
After refueling, stepping up his ladder into the loader and felt pain just behind and slightly underneath left knee.
Ground was frozen and snow covered. Operator noticed blockage in coarse coal chute and moved rapidly towards chute to clear it. Stepped on frozen puddle hidden by snow and right foot broke through ice causing him to fall to the ground. He strained/sprained his right knee and immediately reported the injury to his supervisor. He was sent for medical treatment.
Friday, January 24, 2014, while spotting a customer truck, in the Clean-Fill Area, the employee slipped on a dirt rut and experienced pain in his left knee. Monday, January 27, 2014, he saw a physician and was diagnosed with a knee sprain. He was given Ibuprofen and released to work. February 15, an MRI showed torn meniscus. March 4, 2014, the employee had microscopic surgery.
Technician was repairing machine. Technician stepped backwards and heel got caught on frozen ground and fell back injuring left shoulder.
Employee was putting oil in the Cat 777 Haul Truck. He bent over to get oil and had lower back pain.
While roof bolting, his pin plate got stuck on the ATRS, he reached up to clear it and the plate broke free pinching his right middle finger. ***stitches***
Injured employee slipped and fell on snow and ice in the supply yard at the check in/out time clock landing on his lower back causing a contusion. The employee finished his regularly scheduled shift. The area was treated with limestone chips and ice melt following the incident.

X-rays taken at Davis Memorial Hospital were negative and the employee returned to work on his next scheduled shift. The employee stated he was pulling a hand dolly across the track at the Spice Lick Shaft Bottom and tripped and fell. While falling, he tried to catch himself and fell onto his right shoulder causing a strain. This incident became a medical reportable on 2-14-14 because the employee was seen for a follow-up medical visit that resulted in physical therapy. There were no lost days.

He was evalauted by a physican, released with restrictions and completed his remaining shift. On 1/23 he was evaluated by a specalists and taken off of work.
Management has been made aware through medical documentation that the employee is missing time due to the shoulder accident Jan 21, 2014. On that date the employee alleges that he slipped on ice, fell, and struck his left shoulder against the push arm of a dozer that was being repaired.

While operating haul truck, & while loading under a shovel, haul truck opr leaned over in the seat to grab a roll of paper towels & hit her head on gear lever when a chunk off the bucket contacted truck bed. 8 days later, after experiencing headaches & vision problems, the opr was diagnosed w/a concussion resulting from contact w/gear selector. Mine rec'd documentation on 2/4/14.

While trying to install a bushing into a right track frame on a Komatsu D475 track dozer a mechanic hit his thumb on his left hand with a soft faced hammer. This caused his thumb to smash against the bushing making a cut on the inside of his thumb.
Employee was dismounting operator's compartment of locomotive, stepped on track rail and rolled his left ankle, resulting in a sprain. Employee began missing work on 1-21-14.
EE was trying to unloosen a packing nut from the starter box. EE was using an 18"" pipe wrench and 3' bar to wedge box. While pushing down EE felt pain in his stomach. ""THIS DID NOT BECOME REPORTABLE UNTIL 02/17/2014 WHEN EE BEGAN TO MISS WORK DUE TO THIS ACCIDENT""
The #1 conveyor belt chute at the Ceredo Dock plugged off with coal. Employee was using a sledge hammer to hit the side of the chute on the upriver side of the middle level of the transfer structure to knock loose the frozen coal when a piece of metal about 1/4x10x34 inches in size hit his left hip and back area, causing a bruise/contusion.
Employee was hanging wire mesh and when he pulled it forward on the ATRS he got his right foot caught in the ATRS catching his right foot.

The employee was found laying on the mine floor and was unresponsive to crew members. No cause for the employee falling could be determined. The employee may have a medical. The employee appears to have concussion like symptoms. The employee has a full duty release 1/27/14. Further medical testing is indicated.
Employee states that he and his bolt partner were backing the roof bolter out of the #5 entry. As he pulled up the cable slack back, the bolter turned and began to roll up the cable to the spool. The cable had a loop in it and as it tightened up, his foot was in the loop. This caused a fracture to his (L) ankle.
Employee was making a belt splice. He was down on one knee and went to twisted and sprain his left knee. (He started to miss work on 4/2014 to have arthroscopy on the left knee.)
Employee exited 1/2 ton truck at shop area. When he stepped out of the truck, his foot slipped on ice. He held onto the door handle with his left hand to keep from falling to the ground and injured his left shoulder. The injured miner was seen by an orthopedic doctor on February 27, 2014 and will not return to work until the injury is healed.

The employee at the feeder did not see the car coming nor did the ram car operator see the employee. The ram car pushed the employee down which resulted in a fracture to both his legs below the knee.
Employee was using a pry bar to help remove a pitman arm from a steering box on a Mack truck. The bar slipped causing the employee to trip and fall twisting his right ankle. Employee was taken to E.R. X-rays showed his ankle was cracked in 2 places.

At 12:00pm-1-15-14 he was taken to Reliance.Diagnosed with a thoracic back strain & released to restricted duty. He did not work on Jan 16&17. On 1-17-14 he was terminated for cause. On 1-20-14 released full duty.

Employee was cleaning rock salt off of grizzly bars when his safety glasses fogged up. He removed his safety glasses. A rock flew up and struck his right eye causing a corneal abrasion. Given a Rx for eye drops and released to full duty 1-18-14.
Employee swung off side drill station out to put up first bolt. A rock fell and struck him on the right leg from the knee down resulting in a bruise.
Dismounting from 644H front end loader. As he stepped off bottom rung of ladder on loader, he stepped onto uneven ground & twisted right ankle. Ground was soft due to recent snow melt. Had 3 point contact but all of his weight was on his R ankle at the end of dismount. EE had returned to work 3-24-14, but complications from injury had made surgery necessary.

While working in the area of Tank 5 & 6, EE was positioning a new water chute in place, when he removed the come-a-long on his side and the chute dropped and crushed his right hand.

Miner was climbing down a fixed ladder and lost footing on the ladder. Miner's foot and leg went through the rung of the ladder causing him to twist his knee. Miner was believed to be in a rush causing him to lose footing on the ladder.
Injured states he was removing screen rails when one cut his finger.

Walking in Sugar Run parking lot from personal vehicle to bath house. Stepped awkwardly and twisted left knee in gravel parking lot, resulting in a minor sprain/strain. Dark and raining, however the area was level, well lit, and the gravel contained no abnormalities. Incident cause did not involve training, compliance, equipment, or PPE.
Ee was climbing an access ladder when he bumped his head on an overhead pipe, jarring him and making him lose his grip on the ladder. He fell backwards onto the concrete. He did not lose consciousness. He was evaluated by a physician and diagnosed with a fractured rib.

The employee was removing the charger from a rubber tired three-wheeler and a wire got caught as he was lifting it. This caused his shoulder to pop and he realized he had hurt himself. He continued to repair and reinstall the charger and called a co-worker to bring him out when he was finished. He went to the hospital where he was referred to a specialist, who put him off work.

The employee was investigating a foul odor in the mezzanine above the false ceiling in the pond shop lunchroom. He removed two 8x4 plywood flooring sheets to expose the ceiling tiles. While kneeling down, the employee lost his balance and fell forward through the ceiling tiles to the floor of the lunch room.
An employee was attempting to remove a wellhead from it's position by cutting through the casing with a cutting torch. The employee cut several little openings in the casing and the wellhead fell over and struck the employee due to a lack of support of the wellhead by a backhoe that was being used to complete the job.

Employee received a contusion to his left wrist while trying to catch a ladder he was lowering down from an above work surface.
employee was stacking 100# bags when he felt a pain in his right shoulder. he informed the lead man and they stopped packing, once stopped packing, they informed the plant foreman. they brought employee to get a drug screen and see the doctor. the doctor gave him 2 prescriptions. his report from the doctor states that he can return to work with out restrictions.
Employee was removing rock from a crusher and lost his grip, causing him to fall and land on a rock that was on the floor. This resulted in pain to his ribs. After evaluation at the local hospital it was found that his left lung was collapsed.

While employee was lifting a bag of rock dust, he felt pain in his lower back. He began missing work on 1/10/2014.
Employee was operating a diesel scoop and hit his head on a pin board causing pain in right arm. Employee started missing on 1/13/2014.
Employee pulled away from crusher with his 35 ton truck bed in the air. The bed of the truck hit the ceiling of the mine and although the employee states that he was wearing his lap belt, he was thrown into the steering wheel and suffered lacerations and a broken nose.
Employee was standing on portable steps working on #5 Magnet close loop pump. When employee climbed down and reached last step to ground level, he felt pain radiating inside his right knee.
ICY STEPS, SLIPPED AND FELL LANDED ON BACK. BRUISED BACK AND LUMBAR STRAIN
Employee was struck and rolled down the coal rib of the # 2 entry by the ATRS of the DBT bolter on # 1 unit right side. Employee had been hanging curtain prior to the bolter tramming and he was trying to get to the opposite operators side of the bolter when struck. He was not seen by the operator.
Employee had finished assisting with the replacement of brake shoes on the 1570 dragline. As employee was leaving the area, employee felt a pop in his lower back. Employee was examined by the local doctor. CT scan was taken indicating a lower lumbar strain. Employee could not return to work. Employee was released to assume his normal duties on 01/20/2014.
Employee was using slide sledge to remove bogie pins from D-11 track frame. As he pushed in the sledge, he felt pain in his arm/shoulder. Initial eval. was a sprain and possible tendon tear. Employee continued to work. On 1/24/14 an MRI showed a biceps tendon tear. He continued to work until 1/31/14, when he was taken off work and scheduled for surgery on 2/19/14.
Employee was not wearing a company issued hard hat. Employee hit his head on viper frame. Employee was taken to the emergency room and received sutures.
While examining the return, the employee slipped on smooth bottom covered in red boy. This resulted in a labral tear to the left shoulder which will require surgery to repair.
Slip and Fall due to extremely low temperatures and icy conditions in the yard. Sand had been put down to prevent this, but the sand and snow wore down and this icy patch unknowingly was exposed as workers traversed the area.
He was in the process of placing bolt into pot when he bumped the rotation. A burr on the bolt caught his glove resulting in two facture fingers to left hand.

EE was bending 6ft bolt and EE began to put bolt into hole he said he felt burning in his back.
EE was drilling a hole in the top and a loose piece of scale fell and hit his left hand. Fractured the index carpal bone. He started missing work on 1-9-2014.
Employee slipped/tripped on stairs while exiting office trailer.
Employee was waling by warehouse and slipped and fell due to icy conditions. He has been to the chiropractor a few times but has not returned to work.
Employee was walking down the ramp passing underneath the marginal belt conveyor and slipped loosing his step. He tried to maintain his balance to avoid the fall, he twist his body with force and felt a sudden pain in his lower back. The employee reported immediately to his supervisor and notify discomfort he was feeling in his lower back.
Dozer slide on ice covered slope after stopping which resulted in the dozer rolling over Damage has not been estimated as of the time.
Employee of the 600 excavator was referred ""Fondo Del Seguro De Estado"" for back pain.

The employee stated that while lifting and carrying bags of gunite, he felt pain in his left hip. The employee was diagnosed on 1-13-2014 with a hernia and began missing work. The employee's overexertion and improper lifting was con tributary to this accident.
Employee was walking from mill building to our shop area when he fell on a patch of ice at lab building. He struck his hip and back of head on concrete. He had a CT scan on head & X-Ray on hip, neither showed broken bones or swelling. He went to his physicians a few days later because pain and they diagnosed a bone bruised. He will under go therapy/rehab on hip.
Contractor was in the process of hanging beltline bottom rollers. As he was lifting the contractor felt pain in his back.
Employee was unplugging scoop battery bell when cable arched. He turned to leave the area (a normal movement) and twisted his knee. He finished his shift with no complaints, and came outside. Due to a reconstructive knee surgery from a earlier injury, he requested a report be filled out for precautionary measures.

The employee's left foot got stuck in the mud while closing the Kennedy Doors and fell causing him to strain his left ankle.
Employee was assisting crew which was installing wooden transmission utility pole. Ground was frozen due to 19 degree temperature. When he attempted to roll the pole with a cant hook - his foot slipped on hard ground and he fell. He was taken to ER and X-ray determined that he fractured his fibula.
The employee was walking near the NE Mains power center and fell striking his head and falling to the ground onto his left hand injuring his hand and shoulder. Injured did not miss work until 1/14/2014
The miner was operating the continuous miner when some draw rock fell out form cut. A rock slid out and struck the miner in the right leg. He had swelling of the right knee.
Employee was handling miner cable when a piece of rock 10""x8"" fell from the mine roof striking his left hand causing a fracture. Loose top was not scaled by employee. The injury did not involve any compliance issues. No mining equipment was involved. The accident didn't involve job skills, miner proficiency, training or attitude. No PPE or protective devices on equipment involved.
Employee stated that he was stepping from the bolter to the drill platform and slipped and fell appox. 24""-30"" between the drill platform and the ATRS.

Employee was pulling a hose to the next floor and felt a pain in his groin. On January 21,2014, he was diagnosed with a hernia. He will have surgery on March 3, 2014 and until then has no restrictions.

While building a block stopping employee was standing on a ladder stacking blocks on the stopping wall when he lost his balance and fell approximately 4 feet causing 2 fractures in his left ankle.
Two employees were pulling a SC into the mine with a 25X. They had 2 3/4"" chains between the machines. They went around a corner to backup. When they started forward the chains had slack in them an came apart. The SC rolled back and the hoses pulled the steering valve off and it hit the SC operator in the right forearm.
The operator of the Hitachi EX700 excavator slipped while walking on the track. He fell 43 inches to ground resulting in torn ligaments in his left knee. His first shift missed was on 01/10/2014.
Employee had just repositioned a space heater in the bucket of an IT loader. When he turned to walk away, he slipped on coal covered ice. He fell on his right side over extending his shoulder.

EE was working underground at Alex Energy - Jerry's Fork Mine with another ee. EE was removing a bolt structure from the belt scale, and stated the he felt something pull in his back, as well as feeling sharp pain in his lower back that shot down his right leg. As a result it made it very painful to apply pressure to his leg, especially while standing.
Employee was cutting a small notch out of a roll of belt to determine the carrying side of the belt, which was to be added into the existing belt-line. He was using a Lutz, Model 82 utility knife to cut the notch. As he made the last cut, the knife slipped and the blade struck his left forearm. He sustained a 2"" laceration to his arm which took 7 stitches to close.
Employee was closing open doors on train cars, prying them shut with a bar, when bar slipped out of door, releasing pressure on bar causing employee to stumble, causing pain to shoulder, back, & neck area - On 1-6-14 EE was treated in the ER with a return to work with no limitations. On 2-7-14 EE was seen by a specialist. On 2-10-14 EE would be off work for 3 weeks.
Employee was getting oil for equipment when he slipped on the ice, he caught himself on the tailgate of his truck but he twisted his lower back & hip.

Using shovel to un-jam a conveyor and chute when he felt pain in his shoulder. Case did not become recordable until 2/12/14 after his first medical evaluation.
The employee was down on his right knee placing a plastic seal on a railcar door. As the employee began to stand up, his right foot slipped a few inches on ice causing his right knee to twist resulting in discomfort.

After parking his tri-axle, the employee was checking his tires. He went to kick one of them and his other foot slipped on the ice and it went out from under him. He fell on his back, hurting it and his left leg.

Employee was trying to clear ice from a 3 airline. When the ice blew out of the end of the line it caused the employee to lose control of the line. The line moved backwards out of his hands. The coupling on the end of the line caught the thumb on his left hand, breaking his thumb, and causing contusions to his chest wall and ribs.
Employee was using a cutting torch in the underground shop and a spark fell in left ear. Lost time as of 03/14/2014.
Employee on a step ladder removing a light fixture fell from height of 16 inches and his hand struck a metal container. Debris fell toward him startling him and he pulled back. Swelling, TTP right wrist.
Employee was pulling on a pipe clamp when his foot slipped in loose dirt twisting his left knee. Employee continued working until he went out on surgery 4/11/2014 making it a recordable at that time.
The trained worker was washing out the mixer. He placed his hand in the mixer contrary to his training to brace himself to wash the mixer. In the act of moving the water hose, the hose hit safety switch. This turned on the mixer & pulled his left hand into the mixer. He broke bones in is left hand.
He was using a stone bar to maneuver loose mine track rail on a supply car. While he was prying, he felt pain in his groin area. The cause of the injury was body positioning and overexertion due to poor posture. The miner was not proficient in the use of a stone bar and will be retrained upon his return.
While attempting to pull the latches to disconnect the pole attachment from the Bobcat S250, slipped & twisted trapping knee on the back side of the pole attachment, causing a pulled muscle around the knee.
Employee was exiting the trucker auger and slipped on steps due to ice and snow. He put his left arm out to try and catch himself. He injured himself when he fell across a railing.
On January 22, 2014, Employee was performing shovel cut clean-up work with a Komatsu dozer. Two days later, the employee informed his supervisor that his back felt like it was on fire and he had occasional throbbing pain in his back while operating the dozer on January 22nd.
EE was drilling holes in angle on a drill press to make brackets. He had turned the press off and while removing the angle, the drill bit was still rotating some and there was a shaving stuck to it. This shaving caught his glove and twisted his hand around. He was able to pull his hand free of the glove. The twisting resulted in a broken bone in his hand.
Employee was operating a scraper in the mine. The scraper was being pushed by a bulldozer. The bulldozer encountered a large rock and lost contact with the scraper. The bulldozer then slid off of the rock, and shift forward suddenly, hitting the scraper. This caused excessive force and resulted in a whiplash type of injury to both operators.
EE lifted shaft guide out of skip to put on lump room floor. This caused a hernia.
EE was on the second rung of a A-frame step ladder, step down lost balance and fell to the floor sustaining a fracture to her right wrist.
Team member injured his left knee when he turned while shoveling in a trench.
Employee was on 3rd step of ladder inspecting a sand crew. As he stepped down he missed the 2nd step on ladder falling backwards onto his left arm and shoulder.
Employee was attempting to fuel a CAT 992 loader. As he raised the fuel nozzle to the fill point on the loader, he felt a ""pop"" in his right shoulder. Resulting in an injury to his right shoulder.
Slipped on ice after lunch break. Work area was covered with snow and was not able to maintain footing. Slipped and fell onto back. After a couple of days felt pain in lower back at belt line.

Driving a forklift to empty a trash hopper when he hit a pothole that he did not see causing the forklift to bounce and hurt his back.
Injured employee was working at the bottom of the airshaft when he was struck on the head by a piece of grout. The employee suffered a fractured skull and a laceration that required 5 staples. Compliance with rules and regulations was not a factor. Miners proficiency and job skills may have been a factor.
Employee, was using his lanyard when he slipped off. He then tried to grab to the lanyard to recover himself from falling. While pulling the lanyard he felt a snap on his right biceps.

He caught himself with his left hand, causing him to strain his shoulder. He sought medical attention on 1/15/2014. He has been held out of service pending an MRI.
The mechanic ruptured or detached the proximal tendon of his right bicep muscle while performing a bucket changeout on a Marion 8750 Dragline. The mechanic did not report the injury for four days. On February 10th, an orthopedic surgeon issued restrictions which made this a recordable injury. Physical therapy has also been approved and will be scheduled.
Employee was replacing diaphragms on 260 bag house. Employee was kneeling on his left knee using a pipe wrench to loosen diaphragm housing. While loosening one of the diaphragms, he felt a sharp pain on the outside of his RT knee. The diaphragms are located approximately 3 feet from the walking surface.

Bolting on right side of Joy 14ED miner in 22D, #2 entry, just outby 44 block. Pushed bolt up into finished hole with right hand and rotated body to grab wrench out of storage area with left hand and felt pop and pain in right shoulder and pain in lower back.

The worker was removing lumps out of silo 25 by manually operating the silo valve handles. While rocking the handles back and forth to break up a lump, the valve jammed and the employee felt a sharp pain in his shoulder.
Worker received a small cut on his hand when he accidently hit the valve cover of the excavator that he was servicing. Worker's hand swelled overnight, and he developed a blood infection as a result of the injury.
Developed pain in knee from kneeling to work on truck bed. Did not seek medical treatment until 01/24/2014.
Jammed rock; rock broke loose striking smaller finger of right hand. A cut and broken bone was the result. Corrective action: Cut plates to minimize hang-ups, use pry bar instead of hands.
While loading old drill steel with a forklift at the Vortex ready line a driller was on the ground while the forklift was picking up the steel to be placed on the trailer. The drill steel started rolling off forks and onto left foot.
Employees were moving an air compressor cooling unit with a forklift. While setting the unit down it fell over striking an employee causing a fracture of his right ankle which required surgical repair.
Injured was chaining off a take-up roller when a clevis broke, allowing the roller to move and strike the injured employee on his left leg. This resulted in a Fractured Leg.

Employee was exiting a truck when he felt a pop in his back and pain. Employee was seen at ER and a follow-up appointment was scheduled. On 2/04/2014 at the follow-up appointment the employee was diagnosed with lower back strain and given a restricted duty release.
REMOVING A BELT CLAMP FROM A 54"" CONVEYOR BELT. HE THOUGHT HE HAD HIS HAND UNDER THE ENTIRE CLAMP AND STARTED PULLING IT OFF THE END OF THE BELT. HE HAD HIS HAND BETWEEN THE CLAMP HALVES AND THE TOP CAME DOWN SMASHING THE MIDDLE THREE FINGERS ON HIS RIGHT HAND.
EE was transporting equipment from one level to another up a set of stairs when he felt pain in his lower back.
A mechanic was removing the left side final drive of a D10T Dozer when the male end of the 3/4"" drive slipped out of the magnetic plug. The employee's left thumb struck the dozer's blade arm causing a fracture to the thumb.
Employee was walking up on a berm, to trouble shoot the highwall miner electrical junction box. The snow was deep and the toe of his boot caught a frozen rut. He fell on his left forearm, causing a fracture to the ball in the upper portion of his radial bone.

Employee was pushing on a bag on a scale to settle the material in the bag and felt a pain in his leg. After an exam by an urgent care doctor he was diagnosed with a pinched nerve. The employee was placed on restricted duty for 2 days.
Employee walked into a bent roof bolt plate resulting in a laceration to his left shoulder. Injury required stitches.
Employee was walking to the back of company #749 water truck and slipped on some ice. As she began to fall she grabbed a hose on the water truck with her left hand causing her to strain her left shoulder.
EE was walking from the dragline to the dragline bucket with a torch in his hand when he felt something pull in his right knee.
Employee was walking across refuge truck parking lot. Ground was uneven, snow and ice covered. Employee slipped and fell. He landed on a shovel handle that he was carrying.
Employee was sliding a 55 gallon drum of oil out of the batch to be lifted out of the rack. The employee then felt a sharp pain in the lower back. Employee has taken personal leave to seek physical therapy on 02/05/14 for this injury.

Snow had been cleared but employee stepped on a patch of ice, slipped and landed on his hip, also hitting his head in the fall. Employee taken to physician, placed on restricted duty until re-evaluated. Released to full duty 2-1-2014
Employee was straightening out some 1"" hose, as he was bending down he felt a pop and then pain in his back. He was released back to regular duty following the incident but has since seek further medical attention. He has been placed on restricted duty. Late reporting due to the later medical attention which placed him on restricted duty.
While closing the knife gate, the Porta Power slipped pinching the employees hand between the frame.
Employee was raking the kiln grates. He was pulling a large piece off the grates when he felt a pull in his left shoulder.
Opening feed Lukie on 3-8 flash tank. EE was opening the feed Lukie on 3-8 flash tank using a ""nut runner"" and two multipliers. EE states that when he actuated the nut runner the scaffold he was standing on shifted slightly. The shift caused the nut runner to move where he had it braced with his leg and allowed it to pull on his right shoulder. Shoulder Strain

He went down at an angle and he felt a tingle in the top of his left foot as he fell to his right knee it twisted and his leg was bent underneath him and he felt pain in his right knee.
Injured shoulder while shoveling garnet out of hydrosizer. Went to doctor and was given a injection of steroids and placed on light duty.
Deckhand was loosening barge lines. 2"" rope line fed quickly through his hands and the end of the rope whipped and struck his right wrist causing a fracture to his wrist.

Employee was closing a 4"" plug valve using a damaged valve wrench. The damaged wrench slipped off the valve, resulting in his right thumb getting jammed and smashed against the valve next to the one he was closing.
Employee stated that he strained his lower back while operating a loader maybe 2-3 months ago but is unsure but know he did it at work. Employee stated that the bouncyness in the loader aggregated his lower back.

Employee was removing link arm assembly from loader, he reached to pull the pin holding the arm, when he pulled the pin it moved pinching his hand between the pin and boom of loader. The employees hand was cut requiring medical attention/stitches
Employee was scraping a gasket off of a timing belt cover with a razor knife. The knife slipped and cut his right hand.
Turning primary impact pulley, the index, middle and ring fingers of left hand smashed between v-belt and pulley. Fracture middle finger, contusion, abrasion index and ring fingers. Medical treatment. Restricted work activity.
Employee was installing the inside bolt while drilling his starter. He had his right hand on his rescuer when a rock fell pinching his finger against rescuer. The rock measured 9"" x 13"" x 2"".
on Jan 22, 2014 ee was shutting a pump down for the day when he slipped on wet material striking his left wrist on the pump. He reported the accident on Jan 23, 2014 and was taken to Mercy Health Clinic and was treated for a contusion to his left wrist.
An Electrician caught his left forearm between the fan and the control box located on the east side when the fan assy swung open.

On January 21, the employee was exiting the building through an overhead garage door. He opened the door with the controls and stopped the door just above his head. As he was walking through the door, it began to close and hit the employee on the hard hat, neck, and shoulder. The employee did not seek medical attention until February 17 and dr prescribed light duty for L shoulder.

Employee bent over to pull a bearing housing off of a hand cart when he felt pain in his lower back. Employee was seen by a doctor on 1/23/2014 and was given a restricted duty release.
Employee used a pipe as a cheater bar to turn a valve, instead of asking a coworker or his supervisor to assist. While gripping the pipe and turning the valve handle, the pipe slipped and he inadvertently pulled the pipe into his upper left rib area. Placed on modified duty.
Employee bent over to pick up a cable to hand it to someone. When he grabbed the cable and stood up, he felt something pull in his lower back.

Worker was splitting rock with a hammer and chisel when he heard a popping noise and felt pain in his wrist. He finished his shift and then performed other duties the next 2 work days. When he attempted to split rock on the 3rd work day he had pain in his wrist.
Employee was operating a bulldozer in the mine and pushing a scraper. The bulldozer encountered a large rock and lost contact with the scraper. The bulldozer then slid off of the rock, and shift forward suddenly, hitting the scraper. This caused excessive force and resulted in a whiplash type of injury to both operators.
The employee was removing scale from a valve with a rivet buster. When the scale broke free the rivet buster dropped pinching the employee finger between the rivet buster and the valve. The employee incurred a linear fracture in the distal phalanx.
Employee was climbing out of the operators cab of the Water Truck when he lost his footing on the bottom step. When his foot slipped off the step, he lost his balance, let go of the cab grab rails and fell to the ground. The back of his head and back struck the ground.
Employee went into the Shaker Screen to flip over some gates. He had his hand located at the top of the gate and was shaking out some rocks that had become lodged in the gate. The rocks fell and the metal gate fell on his left hand.

Employee was adding a water sump relay tank to control the level. Employee was opening the suction valve of the second pump when the relay tank overflowed. Employee received burns to his back.

Ee turned off ramp and the truck did not come out of the turn, striking the rib, stopping the truck. The ee hyperextended his thumb. He was evaluated by a doctor and diagnosed with a strain to the thumb, given a brace and released to restricted duty.
EE was working inside a material chute. He was using a cutting torch to re-fabricate the shoot. A piece of slag fell down his shirt. He moved to quick and twisted his knee. EE has a strained MCL.
Employee was stepping down from an elevated platform to retrieve a hose. His left foot sank into mud/fines, causing him to pitch forward and fall to the ground, spraining his left ankle in the process.

Supply Clerk unlocked the double doors to the warehouse filter storage building pushed the lever down, pulled the lever to open the door and the handle broke tearing through his glove and lacerating his right index finger.
Employee was walking up stairs at break time and back muscles cramped up. Employee stated that he felt a muscle pull in his back two days earlier while operating a rivet buster during his shift. Employee requested to be taken to hospital.
EE was carrying the top part of an overcast when he slipped on loose rock and fell causing a fracture to the left hand. He was placed on restricted duty as of 1/19/14 and has a follow-up with Ortho. 1/30/14. EE is employed by MC Ent. Brroks Run os providing this report in accordance to MSHA PPL P-11-V-05
Injured was changing cooler pallets with hydraulic ram and push bar(2.5in x 16in)to push out shafts. Push bar stored in the grating on a vertical pallet. EE was between the vertical pallet and adjacent horizontal pallet. The push bar dislodged and fell approx 4 feet striking EE in the groin. The end of his penis was pinched between the push bar and the edge of the pallet.
Associate was sitting in the loader and leaned over to close the door. He placed his right hand on door frame for leverage and pulled door closed with left hand. He did not move right hand and severed the flesh from end of middle finger.
Employee was doing housekeeping in surplus equipment storage area. He was not wearing gloves while picking up old scrap wood and equipment parts. Employee noticed his wrist itched but was not aware of a bug bite until the next morning when he noticed his left wrist was swollen. We sent employee to the doctor where he received a tetanus shot and a splint to wear for two days.
While working on a Kubota tractor with another employee, the second employee assumed the first employees had completed and shut the bed onto the first employee.

Employee was wearing leather gloves at the time.Was sent to physician, x-rays were taken, no broken bones, no torn skin. Physician placed on restricted duty until recheck 1/20, then released to full duty
Employee was using a bolt cutter to remove a Kirk padlock. As he was cutting the lock, the bolt cutter head slipped and bounced off his left elbow.
Injured employee was climbing up the steps of the crane ladder when his lower leg popped.
Employee states he was manually stacking 50 lb. salt bags onto a wooden pallet when he felt discomfort in lower middle back.
EE was walking on an area of soft ground around the plant when he heard a pop in his right knee and fell. EE has been placed on restricted duty and fitted with a brace. Follow up with Physician is scheduled until cleared for to return without restriction.
Employee was stepping off the bottom stand at 19 PPT, when his left foot slipped off the bottom rung. This caused him to fall backwards on his back at the same time the bottom stand fell on top of his right foot. The stand came down as he tried to catch himself. Contusion left foot
While inspecting dragline 19 bucket employee went to step over a patch of high ground and right foot slid into low spot twisting leg and knee.
Employee stepped back off of a 2 foot step ladder and lost his balance causing him to fall backwards. The employee instinctively stuck out his right hand to break the fall and struck a tool cart that was placed in the area. The impact caused a laceration to the palm of the right hand. Employee did not fall all the way to the floor.
A screen panel was being replaced in a triple-deck screen by two mechanics. One mechanic was working inside the screen loosening the fasteners with a battery impact and ratchet wrench. While ratcheting, the mechanic felt and heard a pop in his right shoulder. A medical evaluation revealed a strained/sprained tendon, and the mechanic was placed on restricted duty for healing.
EMPLOYEE WAS WALKING OFF OF A STACKED SLOPE AND STEPPED/TRIPPED ON A ROCK AND FELL. EMPLOYEE SOUGHT MEDICAL ATTENTION ON 01/15/2014. RESTRICTED DUTY ASSIGNMENT.
Employee states he was removing an arch for canopies and it sprung back, striking him on the left hand. This office was notified 1/16/14, when employee sought medical treatment. There is no lost time for this injury but employee is working modified duty.
Employee was adjusting the primary air on the burner barrel. One of the air cannons discharged and a hot piece of clinker was projected out of the hood door and struck the employee on the left side of his neck. This resulted in first and second degree burns.
Employee was exiting a CAT 980 front end loader. His right elbow struck the upright of the ROPS.
Operator was descending a fixed ladder when left foot stuck in the ladder rung and twisted his knee as he reached with his right leg for the next rung. The operator suffered slight tendon tear and started physical therapy on his left knee 1-27-2014.
Employee was squatting down washing underneath the tail pulley on the 3 deck screen conveyor. When he raised up he felt a pop in his lower back. He had tingling to his groin and both legs.
Employee was positioned about 8-10 feet from two other workers that began to remove the shroud from the dragline bucket using a hammer. Upon impact a shard of metal from the shourd contacted employee cutting through his jeans and into his left knee. He felt a burning sensation and noticed his knee was cut. Employee received five stitches to his left knee.
