Accident type, without injuries

Conveyor that moves coal from the dryer to silo caught fire. The job site was idle & there were no witnesses.

Acetylene tank that was being stored on the shop floor caught fire during an off shift. The tank was not in use at the time of the fire. There were no employees working in the shop at this time.

Employee entered elevator and pressed the 1st floor button. Elevator traveled downward and stopped. Control panel in elevator lost power and could not be moved up or down and the doors would not open. Employee had a power plant radio and called for help. Elevator repair company was called and replaced a failed mechanical relay. Once repaired, employee was able to exit elevator

An unplanned roof fall occurred on the #7 belt at x-cut 58 at spad #13701. The fall was approximately 60' long, 18' wide, and 10' high. The fall will be cleaned up and rehabilitation to the clean-up plan.

An unintentional roof fall has occurred in the #9 Entry at the 6XC of the S1C. The fall was approximately 30 feet in length and 18 feet wide by 14 feet high.

A roof fall occurred in the 11 seam at crosscut 208 in entry 5. The roof fall measured approximately 20'L x 18'W x 3'T. The roof fall will not be cleaned up. The secondary escapeway was re-established in the adjacent entry. The roof was supported with 4' fully grouted roofbolts and date of development was 10/2007.

On 3/28 the mine operator was made aware of damage to the hoist man cage door at 9:03 am with the understanding it would take longer than 30 minutes to repair. A call was made to the MSHA 1-800 hotline at 9:13 am to report this condition.

A roof fall occurred in the 11 seam between crosscuts 207 and 208 in entry 4. The roof fall measured approximately 25'L x 18'W x 3'T. The roof fall will be cleaned up and re-supported according to the approved site specific action plan. The roof was supported with 4' fully grouted roof bolts. Date of development was 10/2007.

The elevator at Portal #9 had excessive vibration when operated. The elevator was taken out of service and the drive motor was replaced. The #9 elevator was back in service at 9:45 pm.

#34 service hoist went down at 6:55 am. Contacted electricians at 6:55 am. Called MSHA at 7:25 am. Electricians found a bad contact. Hoist was back up and running again at 10:20 am. Called MSHA at 10:36 to report hoist was in service.

IGNITION - Occurred on 5 section in the 2-3 cross cut as roofbolter went to spin the bolt and saw an orange flame. The flame lasted approx 20 seconds and was extinguished with powder from a fire extinguisher.

During the 3/25/2018 Sunday night pre-shift, an accumulation of water was found at break 6 from the number two entry to number eight entry on the one left section travel way that stopped travel to the section. All air was travelling in the proper direction.

Electrical components of the secondary egress were below operable temperatures due to the heaters not igniting after running out of propane gas.

A fall of the roof occurred on the 1st NE Mains (1st 54) in the #5 entry, the pillar line fell between crosscuts 26-27. The fall measured approximately 65' in length, by 18'-20' in width, & 7'-14' in height. The fall was in a secondary escapeway entry, the lifeline was re-routed, flagged, mapped, and all persons were notified of the change.

A bulldozer fire occurred at the mine's preparation plant. The fire was caused by a fuel line rupture. The fire was controlled and extinguished using fire extinguishers and water. There were no injuries associated with this incident.

The arm of the drum brake on the hoist drum malfunctioned. The brakes were repaired and the hoist put back in service on Wednesday, 03/21/2018 at 10:45 pm.

Power tripped to one of two longwall power centers in 2R6W section. Power dropped to entire section. Smoke and heat were detected at the power center, all miners were removed to outby the power center, and fire fighting efforts were initiated. Ventilation was re-routed and a fire was extinguished within the power center within 45 minutes of discovery.

Non-Emergency Evacuation due to a contactor failing for hoist controls. Power needed shut off and contactor replaced.

Non-emergency evacuation due to build up on guides causing a slight over amperage in the Koepe shaft. Build up had to be removed so the Koepe could move up and down the shaft.

An unintentional, non-injury roof fall occurred on the 002 Section #5, one break in by spad 3862 that measured 25' wide x 25' long & 10' high. Area was timbered off, bolts have been installed, jacks & beams set & 16' roof bolts installed in adjacent entries.

At approximately 11:15am received notification that smoke was coming out of the top of the #1 Clean Coal Silo at the mine's preparation plant. The fire was extinguished with water directed from the top and side of the silo. There were no injuries associated with this incident.

The encoder shaft that has electronic transmitters to send back to the PLC to let the hoist no its location inside the shaft was damaged and would not allow the transmitters to relay information back to the PLC for the hoist to operate.

One employee in Tilden Concentrator elevator for 70 minutes. Communications with employee at all times, no injuries. Elevator was stuck approximately 8"" below landing. Elevator doors were opened and employee was able to walk out. Elevator contractor Thyssen Krupp conducted investigation, determined brake contact relay was dirty, cleaned and returned elevator back to service.

An employee was stuck on the Cameron elevator when the encoder switch for the elevator failed. The elevator encoder was repaired and the employee was returned to the surface safely. EE was on the elevator for approximately 4 hours before the repair was completed.

We brought up coal mill A and the merrick belt caught fire. Belt burned up and a little damage to belt structure. There were no injuries

A non injury roof fall measuring approximately 7'Hx18'Wx120'L was found near survey station 129 in the right return in an area that had been roomed off. This fall does not obstruct travel or ventilation. All entries to this fall has been dangered off.

Mine electrician was making the daily exam of the #9 Portal elevator when the door switch malfunctioned. The door switch was repaired and the elevator was back in service at 11:10 am. At which point the mine electrician exited the elevator.

Kiln 7 was starting up and just began to produce lime. The hot lime coming out of cooler ignited belt. Belt was burned, wire were burned, and rollers were destroyed.

Dry Ridge elevator was not operational for an extended period of time. It was reported that a wire vibrated off of the outer door switch at the bottom of the elevator.

The breaks on the hoist car were found to be sticking during a regular inspection. The hoist was taken out of service to change the breaks. While dropping the car during the inspection drop a kink in the hoist rope was found. The hoist was again taken out of service to reterminate the rope.

Inspector advised us an inactive coal processing facility had steam or smoke coming from coal pile. We dispatched people to investigate, found steam or smoke, moved equipment to spread material over a large surface, applied water to ensure no fire. Having found no fire we kept applying water to keep down the possibility of a fire.

Employee was traveling off of the section when ee noticed a roof fall in the #3 entry (neutral) in the intersection at 18 break along 3C Belts. This area is not considered an escapeway and is no longer classified as a scoop roadway. No one was injured or trapped.

An unplanned roof fall was discovered at 10:55 am at crosscut 14-11 between the belt and track entry. The fall is approximately 12' high, 15' wide and 20' long. No ventilation or travelways were affected. The area is posted off.

An unplanned roof fall was discovered by a mine examiner at 1530 on March 6, 2018. The examiner reported the roof fall and MSHA hotline was notified at 1537. The incident occurred in an intersection between 2nd South Panel and 3rd South Panel in a worked-out area.

No personnel underground at time of incident. AC Line Contactor on No. 3 Hoist computer system failed, took electricians time to diagnose problem - spare contactor installed and hoist back up and running.

Elevator stopped working with a person inside

The Wana Elevator was being inspected. While the it was moving down, at approx 30-40', the employee tested the E-Stop while in motion. It came to an abrupt stop at approx 3:20am and was stopped until the system could be reset. The system was reset at approx 3:52am and the employee exited the elevator at approx 3:55am. It did not involve any equipment or compliance issues.

Unplanned roof fall discovered reportable on the stageloader between 57 and 58 block of the #3 entry of 4E section. Fall was 12' high, 16' wide and 30' long (approx.). Fall was caused by geologic abnormality of lower cover, beneath a surface stream valley which combined with frontal abutment stresses to cause a localized roof failure.

An electrical power bump shut off fans supplying air to underground. The mine was evacuated. After the power was restored, the electricians were in the process of resetting all power. It was discovered that the clutch system for the #1 hoist was partially clutched-out. The electricians were able to reset the system and restore functionality to the hoist at 06:00 AM.

Between X-cut 41 and 42 in the 13th left return MMU 002-0 a roof fall has occured. Roof Fall dimensions are 16' wide, 50' to 60' feet long, and approximately 10' feet high.

A fall of roof occurred in the 1st North Submain return entry # 2 , crosscut 13-14. The fall was approximately 100 feet long x 18 feet wide and the thickness is unknown.

The belt Foreman & 1 other employee made a belt splice on #7 mainline belt. Belt came on & foreman was dragged approx. 6-7 crosscuts before the belt stopped by a person making belt checks.

Struck against a moving object

A mine mechanic was operating a diesel mantrip along the M5 Travelway when it appears the vehicle struck the coal rib and rolled on its side. The mechanic was partially ejected from the ride and the canopy fell on ee chest.

Struck by falling object

Placing a chute on a screen, the chute became lodged. The employee tried to dislodge the chute with a pry bar. EE was either hit by the chute or pry bar

Caught in, under or between a moving and a stationary object

While inspecting the 4 & 5 truck load out chute repair employee's right ring finger tip was pinched off into their glove. Employee was checking the slack in the cable when a second employee started to raise the chute pinching the finger between the cable & pulley.

While EE was waiting for the continuous miner to reposition, EE moved the shuttle car into the intersection with left arm outside the car. At the same time, another car was in route to the miner, and traveled through the wing curtain hitting the injured's left arm, crushing it. EE was taken to the hospital, transferred to another hospital to amputate the left arm.

Employee was operating a CAT 236B Skid steer doing cleanups under belt lines. Employee stated ee struck something and was jolted forward. EE hand made contact with bumper plate and chest hit control joystick slamming bucket down smashing Right index finger. Finger was damaged extensively and required amputation.

Employee went directly to coal screw area at start of shift to ensure no blockages. Within a couple minutes of arriving at coal screw, employee radioed that ee needed help. Supervisor found ee laying on the grating stating that ee had injured hand. Employee had lost the tip of one finger and lacerated two others. Employee stated ee didn't know how it happened.

Employee was moving stacked cathodes on a chain conveyor, using a vertical bar braced by the conveyor frame as a stop and using the conveyor in reverse to attempt to re align a skewed stack of cathodes - the top of the middle finger on left hand was caught between the bar and the stack of cathodes. The top of the finger was de-gloved, the tip of the finger was amputated.

Miner was assisting in changing conveyor boom on a Joy 14/15 continuous miner when the boom unexpectedly shifted. The miner was caught between the boom and the mine roof injuring miners back. Injury resulted in paralysis below miners waist.

Two employees were in the process of putting the cover back on the tail gate sprocket. They had two bolts to put in yet. 2ndemployee was using the hammer trying to get the bolt holes lined up. Injured employee turned to ask where the bolts were when 2nd hit injured employee's finger with the sledge hammer.

IE severed the left middle finger tip while installing a push arm assembly on a D-11 dozer.

Struck by flying object

Crew was attempting to tram the Highwall Miner while victim in fork loader observed the trailing cable. From the confines of the loader 25 feet away a piece of metal from the Highwall Miner track flew through the slightly opened side glass of the loader striking the employee and causing employee to lose vision in left eye.

Over-exertion in pulling or pushing objects

Initially this was a first aid injury until surgery was required on 3/29/2018. Employee was pushing a camera through pipe. EE was kneeled down pushing hard when ee felt a pain. This incident resulted in surgery to repair hernia.

EE was driving a production truck, and reported a sore back from being shook while in the cab.

Over-exertion in wielding or throwing objects

Miner felt catch in knee when rising from diner table after work. Next day felt knee catch while driving wedge with axe. Next day knee was swollen. Has had previous surgery same location from sports accident. Notified MSHA by phone on 4/3 and sent electronic report on 4/13 and have had no receipt of filing.

Setting a beam in place and it shifted and caught hand.

Changing return roller with hammer. Glove got caught in roller taking hand against conveyor belt. Very muddy.

Miner was stripping a CAT5 communication wire to expose the inside wires and the blade of the utility knife EE was using cut into EE's right thigh causing a deep laceration requiring stitches.

Fall from ladders

Employee was performing repairs on crusher moving from a ladder to the crusher. EE slipped and fell on back. Possible bone fractures in back.

While driving haul truck across rutted section, truck bounced in a manner that caused injury to lower back. Medical treatment resulted in time off/modified work duties.

Fall to the walkway or working surface

Employee was descending an overcast ramp when ee slipped and fell to the ground injuring right leg. EE was diagnosed with a fractured tibia in right leg.

Contact with hot objects or substances

Employee was using a pipe wrench to remove cap from radiator, antifreeze blew on employee's hand causing burns.

Fall onto or against objects

The employee was injured while hand loading material onto a freight elevator. The employee fell forward breaking Rt. ulna and lacerating head when the elevator car floor shifted and employee lost balance. Employee was injured on 3/28/18 was referred to Orthopedic Specialist and returned to work with restrictions on 4/2/18.

Fall from machine

Employee was dismounting a 375 excavator after checking fluids when employee misplaced foot and fell approximately 3-foot to the ground.

Over-exertion in lifting objects

Employee was removing a rock from west main #3 belt line and as they were attempting to place rock in bucket they felt pain in lower back.

Struck by rolling or sliding object

While moving hoses out of the way of excavator on shaft bottom, was struck by rib material, resulting in concussion to employee.

Employee was assisting an injured employee and a rib roll struck the employee in the leg.

Employee had pull cables in hand and went to turn and twisted knee. EE didn't pivot foot when EE turned and it caused a strain on right knee.

Employee was helping to unload an equipment door when EE strained lower left back. The employee worked till 4/2/2018 when EE was taken off work.

Employee was sent to the clinic on March 27th 2018 because of ankle pain. The report from the clinic came back ""unresolved pain causation.""

EE was walking across the drag line bench. EE slipped down. EE states that when EE stood up EE felt pain in the groin. EE was seen at DR the next day. No treatment. EE reports no improvement. Dr listed restrictions that could not be met on 04/12/2018. EE missed first shift on 04/16/2018. Follow up with general surgeon on 04/24/2018.

A fully trained miner was removing the miner cable from a cable handler when a piece a rock fell from the top striking ee in the back of the neck and shoulder. The accident was not the result of any compliance issues nor were any protective guards involved.

Struck by flying object

IE was supervising a planned explosive detonation. When the shot was detonated, a stray piece of rock came through the door of the drill, which had been left open, striking the IE in the back of the legs. This resulted in a distal fracture of the left fibula, and an occult fracture of the right 3rd metatarsal.

Chain being used to pull shuttle car snapped and struck employee in the lower right leg.

Worker was flashed by welder while performing plant maintenance and received a welder burn to the eyes and face.

Was pulling water fill hose and slipped in mud, falling down, felt a pull in lower back. Employee continued to work until April 10th in which employee was then taken off work for surgery.

Employee was walking around a pallet of rock dust when EE tripped and fell twisting ankle.

Contractor was placing the pin in the draw head to connect the locomotive to the rockduster and smashed left middle finger.

Employee was exiting the mantrip bucket and twisted ankle.

Struck by flying object

Injured employee was observing another employee repair an impellor on a Sunflo pump with a chisel when a small piece of metal struck IE on the left cheek. The small piece of metal was removed by a physician on 4/4/2018 when IE also received 5 sutures.

Struck by falling object

Employee stated that as EE was kneeling near the rib on the #1 Section during the last lift of pillaring on the solid block in the #1 entry, a section of rib fell out and struck EE in the lower back.

A repair supervisor was injured after obtaining the location of a hydraulic hose on a haul truck. The location was needed to facilitate the repair of a similar truck. After obtaining the information, the employee stumbled and fell while returning to the step ladder. The fall resulted in a fractured collar bone.

Struck by flying object

The employee stated was installing belt hangers on the helper side of the bolt machine. A rock flew off of the operator side's drill steel, flew across the machine and struck the employee in the face.

Employee was operating a concrete pump truck at a new construction site, the flexible hose at the end of the boom became clogged. Instead of following procedure EE decided to hit the hose while it still had pressure on it. When the clog broke loose, so did the pressure. That release of pressure caused the hose to whip and hit EE in the side of the head.

Employee was hosing shield toes on the F-28 Longwall section during an idle shift when EE got debris in EE's right eye. EE was diagnosed with a corneal abrasion of the right eye and began missing work on 3/26/2018.

Employee was in the process of aligning a knife valve and lost balance and twisted left knee causing pain.

EE was assisting in setting a belt head. The belt head was raised using a track jack. EE was placing a half header between the timber and crib. EE raised the timber with one hand while sliding the half header between the timber and crib with the other hand. The track jack tipped over causing the belt head to sit down on blocking material pinning EE fingers on both hands.

Struck against a moving object

EE was a passenger on a 10-man mantrip. The driver encountered uneven bottom which caused EE to be bounced in EE's seat. As EE bounced, EE struck EE's head against the canopy of the mantrip, causing pain to EE's neck. EE continued working without restrictions until EE was taken off work on 04-04-18, at this time EE began missing time.

Employee stated that while loading a shot at the face, a hole above the employee ignited causing sand to fall on employee's back.

Injured was bent over cleaning crosscut for pumpable crib bags to be hung,a piece of coal(approx.3'x1.5'x6"")fell from the mine roof and struck them in the back. It didn't involve any equip. or compliance issues. It also didn't involve job skills, training, PPE or protective devices.**EE returned to work on next scheduled shift,03/26/18, then began missing work on 3/27/18.

The injured employee was cleaning up some metal and got EE's left index finger caught in between to pieces metal and received stitches.

Struck by powered moving object

Employee had just finished loading a shuttle car and as the shuttle car was leaving the miner it hooked the waterline at exactly the same time as the employee was stepping in the miner cable slack. The slack hooked around a leg pulling them to the ground. The employee was seen by ARH and then by a specialist where it was determined that they had a sprained knee.

While operating a Volvo Crawler excavator, worker was jostled and bounced around which resulted in neck and shoulder pain. The doctor advised EE to miss work for the rest of the week.

Employee was rib pinning on the left side of the miner in 5A section when left index finger got caught between the drill chuck and wrench. Fracture to left index finger.

Struck by falling object

Employee was removing a center beam in the 8 south section from a shuttle car when the beam fell striking EE's hand causing a laceration to EE's left thumb. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Over-exertion in lifting objects

While clearing the metal bin chute, the employee chose to manually move a rock by self and in the process strained right shoulder.

Struck by falling object

The employee was walking with a ram car jumper when they tripped and fell to the mine floor. The jumper plug fell against the employees left small finger. The employee continued to work until 04/03/2018.

EMPLOYEE WAS CHANGING DRILL STEEL. THE DRILL STEEL STRUCK EE IN THE RIGHT HAND WHICH RESULTED IN A BROKEN BONE IN THE HAND.

EMPLOYEE STEPPING UP ONTO HOPPER CAR TO POP THE BRAKE WHEN EE FELT RIGHT KNEE POP.

Injured was driving a mantrip inby on the E-track when a derailment occurred causing the injured to strike EE's left side and face against the frame of the mantrip resulting to chipping of the 8th and 10th teeth and 7 sutures to EE's mouth.

The employee was underground preparing to go a belt drive. On the way to the mantrip, they stepped between the track rails into some mud, which caused them to fall injuring the left knee. Employee continued to work until 06-05-18, at which time they underwent arthroscopic surgery to repair a partially torn meniscus.

A fully trained miner twisted back when EE tripped over a rail and fell to the ground. The injury was not the result of any compliance issues nor were any protective guards involved.

While getting off the train ee rolled ankle. EE was sent to the medic and then sent to the hospital. EE was told to go to Ortho specialist and was diagnosed with a chip in ankle and given restrictions.

A fully trained miner injured left hand when the miner was roof bolting and the drill steel sprung back striking hand. The injury was not the result of any compliance issues nor were any protective guards involved.

Miner stepped into a floor opening, causing injury to R knee.

Struck by falling object

Co-worker was operating Genie manlift inside tank in front and above door sheet working on upper shell vert weld above scaffold. When booming lift up, we think bottom of lift or weld lead caught hand line or scaffold bracket, pulling bracket out of bracket clip. Bracket fell and struck EE as EE was walking into tank through door sheet.

Slips, Trips and Falls: Employees should maintain three points of contact on a ladder at all times. Employee had no outward physical injury but missed one day of work due to concussion.

A fully trained miner injured the left ring finger when bolting the mine roof. A piece of rock slid down the drill steel striking the left hand. The accident was not the result of any compliance issues nor were any protective guards involved.

The employee was climbing a step-ladder when employee fell causing injury.

Over-exertion in lifting objects

A miner was putting hard line ventilation together and felt like they pulled a muscle. Employee went to see the medic who sent them to the hospital where they were diagnosed with a distal bicep tendon rupture. This requires surgery to fix, causing lost time injury.

Walking from Rock Truck to Loader. When EE got to the Loader EE felt a sharp pain in the right leg/ankle. This resulted in a spiral fracture to the tibia.

While raising the belt with a mini-trac to make a belt set up, the belt slid off and came down on employee's head. This resulted in pain in employee's neck and lower back. Employee continued to work until 3/28/2018 when employee was taken off work.

Employee slipped while conducting area inspection, landed with foot under them, and felt pain. Employee was examined by an ERT member and opted to go to the ER where employee was diagnosed with a fracture.

Walking cam on 2570 dragline was being installed and was being set into place by 992 cat loader. Cam seized and was rammed by the loader and jarred the operator, causing whip lash to the operator.

Employee installing roof bolt, while booming up ee hit rotation while hand was still on pin plate and glove caught on pin plate rotating middle and pinky finger, causing fracture to end of pinky finder and chipping bone on middle finger.

Employee was putting top roller in section belt at the back of the lolo. When EE lifted the roller to put it in, EE felt pain in EE's lower back.

Miner was adjusting ventilation curtain when EE slipped and fell onto the mine floor causing injury to EE's shoulder.

Miner was riding the mantrip when it struck a coal rib causing a piece of coal rib to strike a miner in the lower left side back. This accident was not reported to mine management until 03/22/2018. The miner involved worked this shift and the next shift. Miner went to the Doctor on 03/19/2018 and returned to work on 03/22/2018 when they reported this happened at work.

Tree limb had fallen on building. EE and coworker were removing limbs. EE lost balance and fell on right shoulder and right face.

Employee was injured when the #4 belt conveyor EE was crossing started suddenly, which caused EE to fall resulting in a dislocation to the little finger on left hand.

Employee was guiding a barrel over a handrail being lifted by a crane. A strap slipped causing the barrel to drop 2-3"" pulling on the shoulder of the employee causing a sprain/strain.

Employee was loading supplies onto roof bolter when EE stated EE hurt left shoulder. Started as an ache and continued to get worse. Felt a pop. EE worked the entire shift and left for the ER at 3:00 AM on 03-16-2018. EE is off work pending a MRI.

Description: The employee was pulling a piece of belt off of the beltline and miss stepped which triggered pain in right knee.

A group of millwrights were changing a conveyor belt. The millwrights vulcanized the belt and were replacing the scraper. One of Two millwrights carrying the 25lb scraper slipped causing it to fall on the foot causing a fracture

Employee stated that EE was attempting to tighten a huck bolt using a huck gun in the de-slime screen beam. When the employee pressed the trigger to tighten the bolt EE didn't have the gun all the way on the bolt so instead of the gun drawing up on the bolt, it shoved down smashing the fingers of EE's right hand between the gun and the 8"" I-beam below.

EE was bolting top in #1 entry, a piece of rock broke & fell out of the top when EE was pushing up a bolt. EE's back & leg were injured.

During this process, a piece of draw rock slid off of the wire mesh striking EE in the right hand. EE was transported to ER where EE had surgery and is expected to make a full recovery and return to normal occupation within 5-6 weeks.

Employee was riding on the back seat of the shuttle bus when the bus operator abruptly hit the brakes due to holes in the road. This caused the employee to be thrown out of the seat and strike the seat in front of EE. EE's left leg was caught under the seat in front of EE and EE's weight went over the seat. This incident became reportable on 3/27/2018.

Employee and another worker were lifting a shuttle car tire when employee injured left hip and leg. Employee was taken to the hospital and has not returned to work. Employee will receive an MRI to diagnose.

EE was first injured on 3/12/18 but never reported the incident. While squatting to lift a tipped over 50gal drum EE felt pain in the groin. On 9/5/18 EE first reported the incident. EE waited another month before seeking treatment. It was discovered EE had a hernia. The operation was performed on 11/19/18. Due to the delayed nature of this injury it is now being reported.

EE refused ambulance and traveled via POV to LRMC ER where EE had a X-ray and found that EE received a broken fourth distal digit. EE was given 3 days off work and referred to a specialist.

Employee was operating a front end loader. The loader hit a bump in the road while approaching a stockpile, which caused the operator's head to hit the sun visor resulting in a laceration. The operator returned to work without restrictions on March 19th.

Miner/Supervisor was assisting with a screen change on the 6x20 portable screen plant. Supervisor was exiting the screen walkway and stair when they turned and missed the stair and fell to the ground.

Employee was prying on the lip shroud of a drag bucket. When the bar slipped, they strained their right shoulder.

EE was carrying a belt roller cradle, while doing so EE tripped causing EE to drop cradle onto top of foot. Cause of this incident was lack of employee concentration. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency was a factor.

Employee had just parked the CAT 980C loader. As ee was exiting the loader, ee accidentally stepped on a wheel chock that ee had not noticed when parking. The chock was located at the bottom of the loader's step ladder. Stepping on the chock caused leg to twist, resulting in injuring left leg.

Helping load a screen bearing in skid steer loader bucket. Bearing rolled pinching finger against side of loader bucket. Went to hospital some stitches required.

Injured took winch hook off bed plate and while walking away it shifted 3-4"" and struck injured in the back of the right calf, resulting in a fracture to fibula.

Walking down the track side of 11 belt near 28 block and struck head (hard hat) off of the mine roof resulting in an acute sprain of ligament of neck.

EE was assigned duties as a blaster. While backfilling coal blast holes, EE felt a pop and pain in right shoulder and neck area. EE returned to work and continued to have pain. EE was evaluated by a doctor and was unable to work starting 03/15/2018.

Ee was moving metal fittings with two coworkers. As EE bent down and began to lift, EE felt a sharp pain in EE's low back. EE stopped the lift and was taken to the ER for evaluation. EE was diagnosed with an acute muscle strain and spasm, and was released with restrictions that the site could not accommodate.

Employee was moving the piece of bracing (5 ft, 46 lb). When EE put it down, it slipped and pinched EE's right hand middle finger against another bracing resulting in bleeding abrasion. Accident was witnessed by co-worker. Employee was transported to ER for treatment.

Employee noted back pain at the end of shift after performance of normal duties. On March 12th, the employee was cleared to return to work without restrictions.

Unloading rollers for 11 belt from the passenger compartment of a diesel mantrip parked on 9 south haulage near 11 drive and felt pain near left scapula and right rotator. Diagnosed with strain to soft tissue of left scapula and right shoulder.

While roof bolting a rock fell and struck the hard hat of the EE, pushing hat onto EE's glasses, which caused glasses to lacerate EE's nose.

Employee was repairing worn spots on a D-11 dozer blade. While building up the blade, a piece of molten metal fell onto the top of employee's left boot and burned through the leather. The diagnoses was a full thickness burn to the left foot.

Employee stood up after opening a rail car gate and felt a ""pop"" in knee. Employee initially refused medical treatment. Employee requested medical treatment on May 9th at which time surgery was recommended. On June 4th 2018 employee went off work for surgery.

Was leveling a roll of belt that was suspended from the roof with a 3/8 in. chain. The belt shifted and smashed EE's hand between the belt and chain.

While walking EE slipped on a small patch of ice that had a light coating of snow. EE fell, landing on their right arm. EE had pain in right shoulder.

EE reported having discomfort in shoulder from attempting to connect fast fuel nozzle on T-71. Seen by site responder and worked rest of shift. Continued complaints of discomfort lead to further treatment. Had surgery to shoulder on 5-9-18.

Employee slipped and stopped self from falling by bracing self against the kiln wall. EE's left arm struck the kiln wall causing a fractured humerus.

Placed hand by rack and left tip of middle finger was smashed.

The employee was struck by a hose. The employee received a minor abrasion to the bridge of EE's nose and an alleged concussion as a result of the incident.

Employee was on a section hanging mesh when the rib rolled knocking the roll of mesh into EE's right ear causing laceration requiring sutures.

Employee was cleaning off the top of a shield with a slate bar in the F-27 Longwall section. While prying down on the slate bar EE caught EE's left hand/fingers between the slate bar and the handrail. Resulted in a fracture to the tip of EE's left middle finger.

At the end of the day, the employee stepped out of the loader carrying lunch box. When EE stepped out, EE slipped on the icy platform on the loader. EE was able to grab the handle but struck left side on the machine, fracturing two ribs. Cold temperatures, rain and ice was forming throughout the day.

Employee was carrying crib blocks when ee felt a pull in stomach. Employee did not use good lifting/carrying techniques and over-exerted self. No mining equipment was involved and employee has proper training. Received information on 4/2/18 that employee has been diagnosed with a hernia. Employee has not missed any work due to this accident at this time.

EE stepped up on the rear work bumper of EE's truck to turn welder off. When EE stepped back down EE missed the step and fell to the ground which was less than 3 feet and landed on left elbow causing it to dislocate.

Employee was repositioning an apron feeder guard. After some time, the employee developed pain in right shoulder. The employee received a diagnosis of impingement syndrome, AC arthrosis and a rotator cuff tear.

Loader driver drove over a large dip in ground, front of loader pointed downward, at same time tilting bucket up and down to shake remaining material out, lower edge of bucket stabbed the ground, bringing it to an immediate stop. Driver hit head on the windshield. No damage to loader. Driver had headache; slight bruise on forehead, no cut, no swelling.

Employee was cleaning material discharge from the dust chamber, using a skid steer when additional dust discharged from chute burning employee. Employee exited skid steer, landing in material pile also resulting in burns.

EE had stopped to take a restroom break and while on the ground, returning to haul truck EE lost balance and fell. EE hurt thumb on the ground while trying to catch self. EE sustained a severe sprain to thumb.

Injured employee was dragging welding leads behind them when they became lodged under a stationary tire. This jerked employee's arm and injured the shoulder.

Employee was unloading material from the back of the plant truck when employee felt pain in groin. Employee was diagnosed with a hernia. This was not a restricted duty or lost time injury until employee had surgery on 06/15/18.

EMPLOYEE WAS WORKING AT 517 SHOVEL AND IN ROUTE TO THE VEHICLE CARRYING TWO ROLLS OF WIRE WHEN SUDDENLY THE HIGH WALL NEARBY BEGAN TO FAIL. EMPLOYEE STARTED TO RUN AWAY FROM THE IMMEDIATE DANGER AREA AND DID NOT NOTICE THE TRUCK WAS BEHIND THEM. WHEN EXITING THE AREA THEY RAMMED THEIR SHOULDER INTO THE VEHICLE. Installed engine and removed turnbuckles from lifting jig. Lifting jig got hung up on horse collar. Wiggled jig with both hands to try to get jig free. Jig started to move but got hung up again. Wiggled it and jig moved sideways, as I was moving out of the way of the jig it popped loose moving very fast and caught my right hand pinky finger.

Employee went in to collect seal samples that needed to be tested. EE drove in to 9 East as far as ee could, then lifted the box & carried it to truck. EE didn't seek medical attention until June & was diagnosed with a bilateral inguinal hernia on 6/14/18. The mine was notified of the June 28 diagnosis as work related on July 6.

Over-exertion in pulling or pushing objects

Employee was closing the east side store room dock door on March 21, 2018 when the door jammed and caused the employee's left arm to jerk. Employee continued working without issue but subsequent medical visits revealed a rotator cuff tear. Surgery was deemed necessary and performed on June 25, 2018.

Injured party states EE was grinding on cab shield in truck bed. After EE was done EE was climbing out through tailgate and ladder that was propped up kicked out from gravel and injured party landed on EE's right side bruising EE's ribs on bed of dump truck.

A raise miner pulled on the air and water line which had become stuck. Miner repositioned and pulled hard on the lines and felt a pop and a burning sensation in groin area. Miner reported the incident and maintained was OK. Miner was taken to the doctor where it was determined miner had a hernia, recommended miner see a surgeon to determine treatment.

EE is a Splitter Operator at the Chilton Quarry indoor within the Chilton Manufacturing Facility. EE was assembling boxes. EE put a nail in one section followed with a second nail. The nail gun then jumped and landed on top of EE's left hand. When the gun hit EE's hand, a nail fired into the top of EE's left hand as EE's finger was still on the trigger.

Injured employee stated that ee fell from the deck of a rough terrain crane when it became unstable while the Operator was moving the boom as it was chained to a transport trailer.

Employee had just climbed off of the bulk truck and slipped and tripped over some round grinding balls and fell to the ground injuring EE's right shoulder.

The employee stated the ee was returning from the south seals when ee stepped on a rock that was laying in a water hole. The employee immediately came outside to seek medical attention and was placed on restricted duty and on 3/16/2018 employee went back to the hospital and was take off work. The incident is being reported as a lost time.

Employee was welding by work bench and a piece of metal became lodged in left eye. Employee went to dr on 3/15 and a piece of metal was removed and EE was given an antibiotic.

EE didn't go to the doctor until Monday, April 2, 2018 due to the fact they were hoping it would get better over time, but it actually became more sore and agitated as EE continued to be active on the leg. EE notified HR that EE wanted to seek medical attention on 3-26-18.

While trying to line up the bores of the frame and the rod of the cylinder, the bearing on the frame was not lined up and was caught. While lining up EE's hand was on the bottom of the cylinder, when cylinder lined up it dropped 3"" onto EE's finger. EE had gloves on at the time.

The scoop operator stated scoop was sitting still when the injured yelled you got me. The operator did not realize they hit employee. When interviewing the injured employee they said the scoop swung over towards them hitting the top of their head.

Approx 7-9 away from saw a small piece of stone ricocheted from the saw cut and struck employee approx.1"" under the hard hat brim in the back of the head resulting in 2.75cm laceration. Roof Bolter operator (IE) was attempting to knock a few pieces of rock from the already installed mesh with a bolting steel. A small rock fragment struck lip causing a laceration that required stitches.

EMPLOYEE STATED THAT WHILE WALKING INTO WORK TO THE CONTROL ROOM, SOMETHING FELL INTO EYE. EE WAS TREATED BY LOCAL EYE DOCTOR AND A PRESCRIPTION MEDICATION WAS USED IN EYE.

Over-exertion in lifting objects

Lift rock approx. 75 lb to place into tumbler.

Some paper fell behind a filing cabinet drawer. While employee was retrieving the papers, they placed their left hand on the cabinet drawer slide. A piece of sharp metal lacerated the left ring finger.

The employee was working on the continuous miner cat when one of the cat pads flipped over and pinched their finger, resulting in a small fracture of the left litle finger.

Employee was using a chain ratchet when the hook broke and caused the chain to hit employee in the right elbow resulting in a laceration which required 3 sutures.

Employee was disassembling steering shaft to replace u-joints using electric impact gun and wrench to remove retainer bolt. The retainer clip spun around catching EE on EE's left thumb causing a cut requiring stitches

Injured employee crushed left little finger between the flat bed pickup rail and replacement dragline bucket tooth that ee was loading into the truck bed.

Stepping off truck employee went to plant foot on ground and reposition hand on railing, leaning back hand came off railing only one hand left on railing and fell backwards and hit head on ground

The employee stated EE was a dropping miner and waterline cable together when the rope hanger swung back and caught the employee in the left nostril causing a laceration. The laceration required three sutures.

Employee was removing wrapping tape from the seat of new conference chairs. Employee gathered paper to cut and remove, and cut the end of left middle finger with scissors. Skin was lacerated on three sides of the tip, remaining intact on the side closest to thumb. Employee received 6 stitches to aid in healing of the lacerated skin.

was blowing out plant air compressor radiator with air hose...this location is contained within a small room...some debris generated by this process got behind safety glasses...washed out but believed some remained...ER staff found nothing and blue light revealed nothing...prescribed anti-biotic as a precaution and released to return to work with no restrictions

EE (injured Miner) and another Miner were hanging curtains in the 8WFWSWM area. EE was holding the curtain while the other Miner was using a hand held drill to drill a small hole in the curtain, so they could pin it to the wall. The drill caught the curtain and ee glove, spinning the glove and hand up in the curtain. ee right thumb was fractured.

The EE was working to remove a bearing housing from the Wobbler Feeder, located at the primary. Upon removing the bearing housing from the shaft, the housing slipped, catching the employee's hand on the way down, catching the employee's index finger between the housing and the feeder structure, causing a rupture wound to the finger that required 19 sutures to close.

While using a 9 inch grinder the cut off disc broke apart and a piece of the disc fell into the employees right eye.

Injured had a foreign body in eye which caused a scratch to eye. Injured was given a prescription for the injury.
