Accident type, without injuries

One miner was entrapped on the miracle run elevator for approx. 2 hours. The rails the car runs on experienced seasonal expansion which caused the safety circuit to trip on the car. Schindler elevator services corrected the issue.

An unplanned roof fall was discovered in the 6 North Right Return at 12.5 block. The fall was approximately 25' long, 17' wide and 6' high. The fall was most likely caused by gradual deterioration of the entry. No problems with mining machines, PPE, training or compliance issues.

An unplanned roof fall on the off side of the belt in a crosscut was discovered by the examiner during EE's examination. It did not impede travel or airflow. There were no injuries. The roof fall measured approximately 90'L x 20'W x 6-8'T.

No. 1 Shaft hoist operation was interrupted due to programming/electrical issues. Evacuation was initiated immediately. Hoist operation was restored at 2:10 PM.

At approximately 11:10 p.m. an outby examiner found a roof fall in the intersection of the old scoop roadway at 23 break on 3C belt. No one was injured or trapped. Both agencies were notified and all approaches were cribbed and dangered off.

No. 1 Shaft hoisting operation was interrupted due to programming/electrical issues. Hoisting interruptions occurred between shifts. Shaft hoisting operations were restored at 6:25 PM.

At approximately 3:10 p.m. the service hoist went down while performing the daily checks. It was determined that an e-stop switch on the bottom of the shaft had went bad. The e-stop was replaced, retested and the hoist was put back in service. No one was injured or trapped. Both agencies were notified.

There was an unplanned roof fall in the #11-2nd Main West Parallel X-21.

The mine experienced a roof fall outby the 3 North Section. The fall was located in the #3 entry (belt line), between 24-25BLK. The dimensions of the fall were 90ft x 17'9"" x 8ft. There were no injuries from this incident.

At 7:30 AM an articulated haul truck crossed a dike which caused the road bed to fall into a hole under the road which is located on the top of the dike. At 10 AM the secondary wash plant shut down to reduce water entering pond 2. At 2:15 PM the diameter of the hole changed so was reported to MSHA as an unstable impoundment.

A high voltage fuse on a power pole controlled by the utility operator blew. The blown fuse continued to spark and burn. The utility provider was contacted but as it took longer than 30 minutes to arrive, notification was made. No injuries or damage resulted from this.

IGNITION - Occurred while using a cutting torch at the north bunker to remove a metal I-beam. When EE went to light the torch it ignited methane there from the coal material. The ignition last approx. 15 to 20 seconds and extinguished with water from water hose.

At 4:22 am it was reported to the mine dispatch that flames were observed at the top of the shaft headframe. Fire was extinguished in 16 minutes of discovery, it was determined the main bearing for the skip sheave wheel had failed resulting in the heat source for the fire.

Cat 777 haul truck delivered load of stone to new road construction site. The truck backed up to the edge of a shallow trench, which collapsed, causing the truck to tip over on its right side. The driver could not exit the truck due to the height off the ground and dismounted using a portable ladder 45 minutes later.

Roof fall in 207/208 block, Number 2 entry. Fell on the Continuous Mining Machine when Miner operator was removing Miner from area. Miner had to retrieve with hydraulic retriever.

Maintenance employee was entrapped in the old finish mill passenger elevator for an hour. Employee had a radio and was in good health.

#2 Hoist began to hoist crew from 4900 level of the mine and the hoist would not build amps. The cage slowly lowered and stopped approximately 25' below the station after multiple attempts by the hoistman to raise the cage. 3 individuals were on the cage at the time and were not able to exit until the hoist was repaired.

The Dry Ridge Elevator was inoperative for a time due to an electrical fault that was reset.

On 4/16/18, a miner working in an Alimak stope could not exit the stope and exceeded the 30 minutes after their partner took the climber down and was not able to make it back up to the stope level with the Alimak climber due to low air pressure.

Examiner found a fall in the 65 cross cut at 1st North #4 belt 5-6. No damage to equipment and no persons affected. Management called the MSHA hotline at 4:05p.m.

An Examiner was traveling to MMU 001 section and found a fall in the #3 entry spad #12+60 on 5 belt. MSHA Hotline was called at 1:15 A.M. to report the fall. The fall damaged some belt structure and the outside structure of a VFD drive.

A lightning storm caused a break in power that led to the coal mill on Kiln 2 going down. While trying to restart the coal mill fan, the attendant noticed a issue with the starter. An electrician was notified to report to the plant to check the starter. At approximately 12:45am the coal mill expansion joint caught on fire.

An unplanned roof fall occurred on the #4R pillar section. There were no injuries or equipment damage due to the fall and no escapeways were blocked. The section will be pulled back 9 blks where mining will resume

During preshift exam a roof fall was discovered on the 2-East belt inby crosscut #29. The fall measured 16' by 16' and was 6-8' in thickness. No persons or ventilation was affected. The area was previously supported by 6' torque tension and 12' cable bolts. The area was cleaned up and re-supported.

Shot crosscut into intersection at 10:00 pm 4/13 at shift end. Fall happened sometime between 10:00 pm on 4/13 and 8:50 am on 4/15 when first signs of a possible fall were discovered, confirmed by foreman at 9:05 am. No damage or impairment. No production crews were underground on 4/14 or 4/15. First evidence of a fall discovered by crew checking water pumps on 4/15.

Sustained high wind of 45 MPH caused 7200kv phase to ground or 12.5kv phase to phase power lines to arc and catch dead weeds and trees on fire below. Area burned was about 2 acres without any damage to equipment or structures.

A fault of the governor slack switch and a rope gripper fault occurred on the Sparta elevator which interfered with the use of the elevator for more than 30 minutes.

The main service elevator encountered an over travel fault. The sensor was repaired and the elevator began normal operations at 3:06am on 04/16/2018.

The No. 1 phase on the East motor of the Nordberg Hoist went bad. Maintenance had to braze the bus bar to get it back in working condition. Tested and returned to working condition the following day 2nd shift.

A roof fall occurred in the 9 Seam panel 4 entry 5 between 101 and 102 crosscut in the inactive workings. The roof fall measured approximately 70'L x 9'T x 18'W. The roof fall will not be cleaned up. The roof was supported with 5' fully grouted roof bolts. The date of development 3/2018.

REPORTABLE DUE TO ROOF FALL - 1 West belt line at crosscut 6 top fell above support for approximately 20 feet wide 20 feet long on belt line and 2 feet above roof bolts. Entry was dangered off at all crosscuts and double header row set in crosscuts, no airway blocked, no escapeway blocked, no injured employees.

A fall of the roof occurred on the 2nd SE Mains, #1 Unit, crosscut 148, entry #2 (intersection). The fall measured approximately 20' length, 18'-20' width, & 7'-15' thick. The fall has been cleaned up according to our approved rehab plan, additional support has been installed in the area, with the fall area being arched.

There was an unplanned roof fall that didn't fall above anchorage but did have the secondary escapeway blocked.

A kink was found in the Crabapple hoist rope during regular inspection. The Kink was found 16ft from the hoist car. The Hoist was taken out of service and reterminated.

A contractor vehicle caught fire while in a blasting pattern and was not extinguished within 30 minutes of discovery.

An unintentional roof fall occurred in 3 East Mains in the #0 Entry at 2 Wall intersection. The fall measured approximately 8-10' high by 16-30' wide and 90' long. There were no injuries as a result of this incident. *Conditions contributing to the roof fall include adverse geological conditions in this area.

An unplanned roof fall occurred in the belt and track entry on the #2 section 3 X-cuts out by section F8. The fall will not be cleaned up. It will be crib and dangered off and move the section.

A customer driver was fatally injured when the driver's over the road truck ran over the driver when driver fell out of the truck at the check in station.

Caught in, under or between running or meshing objects

Employee was in 6W in the process of handing a roof strap to bolters when the miner oper sumped head back pinching employee's L hand between the rib of #3 entry and the stored roof straps. The accident resulted in the amputation of the left middle ring finger at the mid knuckle. Cause of injury was awareness. No other pt 50 criteria applies.

EE & co-worker were tasked w/applying herbicide to plants & were using ATVs to access the site. EE became stuck. EE walked up to co-worker's ATV & tried to play out the winch line. After finding it wouldn't play out EE placed middle finger of lt hand in winch line hook & asked co-worker to run winch out. They didn't use remote winch controller & EE didn't use safety pull strap.

EE went on break, noticed dusting in main packer. Went to MCC panel check that dust fan was on (seen that the dust vane & #6 screw were off like they're supposed to be). EE went to dust vane visual inspection port, removed the inspection cap to see if product was flowing, stuck hand in port, while pulling their hand back out caught their fingers tips on the vane fins.

Employee was assisting in mine belt recovery; placed a hand on sheave rope and turned their head, at this time the left pinky finger made contact with the sheave pulley which resulted in an amputation of the left pinky finger at the first knuckle of the finger.

Miner reported a pulled muscle working on a RAL. TQM followed up with Miner @4 P.M. to complete incident report. Miner stated ee took aspirin for discomfort, no further first aide was requested. Miner called off on 5/1 and went to Med-Express, received steroid shots and 800 mg of ibuprofen and a DR note not to work on 5/1/18.Miner did not follow procedure for workers comp.

Employee was loading cable into scoop bucket and foot got smashed under bucket causing injury.

Employee was told not to cut filters. At approximately 11:50 am employee took a used fuel filter that had been taken off a machine and using a razor knife, attempted to cut into the filter to take it apart and see the inside. The razor knife slipped and employee cut left pointy finger requiring a few stitches.

While carrying a can of hydraulic oil, miner stepped in a hole in the mine floor causing the miner to twist EE's back.

While operating the roof bolter, the employees drill steel fell to the mine floor while the drill boom was being lowered. The drill steel got caught underneath the drill boom, pushing it into the employees right foot, causing a non displaced fracture to the 3rd metatarsal.

A fully trained miner was installing glue into a bolt hole when a hydraulic hose fitting burst spraying hydraulic fluid into miner's eyes. The injury was not the result of any compliance issues nor were any protective guards involved.

Caught in, under or between running or meshing objects

Operator performed an unsafe act by placing left hand in a machine to remove wet material after SS instructed EE not to because the machine was in start up mode. The Operator put hand into the discharge chute & the machine started up and cut EE's left pinky. First aide was given until an ambulance arrived. Operator was referred to plastic surgeon later that afternoon.

Employee reached for a drill steel while bolting off of the miner and felt a pain in shoulder. Employee continued to work all scheduled shifts until employee was taken off of work on 7/9/2018 to repair injured shoulder.

Struck by falling object

Employee was taking a ventilation tube to the face of the CM section when head coal fell out hitting employee on the head. EE's hard hat was knocked off and a smaller piece hit the top of EE's head causing a laceration. EE was later diagnosed with a concussion & began missing work. No rules or regulations were violated and employee is properly trained. Was wearing required PPE.

When EE finished cleaning the man basket EE grabbed the boom with left hand in latch area. It caused the pin to slip in place catching EE's left thumb between the latch and boom.

Employee states while moving miner cable on #2 unit stepped in hole of mine floor twisting and straining left calf muscle. Finished shift and returned to work next day, did not become lost time until 5-1-2018.

A fully trained miner was lifting a cable when they felt pain in the left shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

Struck against stationary object

After conducting an exam in the tailgate ee was crossing over the gob plate when left foot slipped. This caused weight to shift and ee grabbed onto the gob plate with left hand/arm to keep from falling injuring left shoulder. On 07/19/2018 ee had surgery to repair left shoulder.

777 haul truck operator was jarred around in the cab due to excavator loading the HT made contact with headache rack. Employee was checked and released to full duty. 5/1 employee reported symptoms what was thought to be a personal medical. On 5/11 EE received restrictions may be linked to 4/26. Employee has been referred to a neurologist, results are forthcoming.

Struck by falling object

Employee was shoveling along 5 South 1 belt, when a rock fell from the mine roof striking employee on the head and shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

While constructing overcasts, EE received a small puncture wound to left hand resulting in subsequent infection. This injury was not reported to the insured's employer until 5/7/2018.

Employee was loading tray on bolter when EE struck EE's right elbow against the bolter resulting lost work days.

Site inspection on 4/27/18 showed 3"" of slightly loose soil conditions under the left track. The right track had solid ground conditions. The excavator was removed from the lake on 4/28/18. Inspection of the excavator showed the bucket to be 3/4 full of shot rock and curled almost up beneath and between the tracks. It appears the operator undermined self.

PM Tech was servicing D10R dozer when the Tech lost balance. Grabbed ahold of dozer with right arm to regain balance. This resulted in twisting of left knee and feeling a pop in right shoulder causing pain to both.

Employee was moving steel track ties out of the track entry into a cross-cut when right foot caught on the mine floor causing EE's knee to twist and EE fell to the mine floor. Employee didn't violate any rules or regulations, was wearing required PPE and is properly trained.

Stepped in hole on surge belt and hyperextended right knee; employee did not start missing work until 08/31/2018.

Struck by flying object

Mechanic was installing track pin with the 5P2380 track tool unit when the ram adapter fractured into 3 pieces. The mechanic was contacted on the right side of the face resulting in lacerations and fracture to the jaw.

Struck by falling object

Employee was side bolting on the continuous miner when a piece of rock ( 16"" x 6"" x 2"" ) fell and struck the employee in the right hand. The employee continued to work until 10/25/2018 when they were taken off work to have hand surgery.

Employee stated that EE was operating the dozer on the Raw Coal Stockpile when EE felt a pain in neck. Employee stated that EE felt pain when EE turned to look behind EE while backing the dozer up.

Struck against stationary object

Employee was sitting in a chair in the AA room of the assay lab, when employee pivoted in chair and struck knee cap against the corner of a filing cabinet. Knee was contused.

EMPLOYEE GOING UP LONG STAIRWAY TO TOP OF TANK TO CHECK LEVEL AND EXPERIENCED A POPPING OF LEFT KNEE. THIS ACCIDENT HAPPENED ON 4/25/2018 BUT EMPLOYEE DID NOT GO TO DOCTOR UNTIL TODAY 5/09/2018.

Struck by falling object

While off loading a trailer a co-worker pulled on a pallet, containing sacks of aprox. 300lbs., with a pallet jack. This caused another pallet to shift resulting in a top sack falling onto the injured workers back.

Employee stated that EE strained EE's shoulder while using a slate bar to pull a coal rib. Employee continued to work until July 16, 2018 before going off work due to the injury.

Fall from ladders

Climbing off a ladder that was leaning against machine, ladder slipped causing ee to fall to ground injuring right knee.

Fall to the walkway or working surface

Operator was cleaning out crosscut from track to 3 belt on 1 north at 15 block. Injured was pushing wheel barrow of material that tilted causing ee to fall down and strike knee on the mine floor, resulting in a contusion.

Employee had been kneeling and felt a pop in left knee when ee stood up.

The miner was repositioning a 50lb bag to be re-inked and twisted back resulting in a muscle spasm.

The employee was loading bags of ammonium nitrate when they bent to lift, felt a severe pain in the lower back.

Employee used a lifting bar with co-worker to lift a truck box pin. The employee felt a strain in lower back.

Struck by falling object

Hanging slick line 1 block inby end of track in 5E, line was laid out and end was hung up so that a scoop could travel under it. The end of the slick line unrolled out of the hanger and hit the injured in the mouth, resulting in a facture to the alveolar ridge of maxilla and subluxation of the upper front teeth

Employee was troubleshooting a belt sequence cable on the 7 North Belt when the employee stepped in a water hole and twisted left knee. This employee has continued to work until 5/11/18 when the employee began missing work due to this injury. Employee did not violate any rules or regulations and is trained and experienced in job.

EE was pulling in to the 79 LOM with a loaded truck and struck a rock with front right tire causing EE's head to contact the roof of the cab. EE was transported to the hospital and diagnosed with a concussion and neck strain.

Employee was climbing off a Cat D8 that was in the shop for repair. When ee stepped off the machine, ee rolled ankle and had a hard landing on the shop floor. The following day ee went to the doctor's to have it examined. EE has a broken fibula and torn ligaments.

Employee was cleaning a piece of metal with a hand held angle grinder with a wire wheel attachment & as EE worked the grinder back and forth on the metal surface the wire wheel caught a headlamp strap hanging out of EE's pant pocket and pulled the grinder into EE's groin area. Seen site medic, referred to ER, restrictions were issued.

While backing the roof bolting machine down the no. 3 entry the cable reel was taking up cable causing the trailing cable that was hung to be jerked down striking the miner on the head.

While removing a bolt on the ""lip saver"" of a 993 K model loader, a bolt with a rough edge cut the glove the employee was wearing causing a laceration to the outside edge of the right palm.

WORKER WAS STACKING CHOPPED ROCK ON A PALLET WHEN RIGHT HAND PINKY FINGER GOT CAUGHT BETWEEN TWO ROCKS. RESULTING IN A SMASHED AND CUT FINGER.

Fall onto or against objects

While working inside C-1 chute employee alleges ee was hit on the hard hat by loose material causing ee to fall backwards and hit ee head on the chute.

Employee stated that EE was carrying material and going in and out of man door and at some time during the shift EE strained left rib area. Employee did not start missing time until 05/01/2018.

Employee had gotten off of forklift to move steel plate back on forks that had fell off and felt pain in right shoulder and bicep area.

Employee was operating shear on longwall and as ee was walking following the shear foot was caught under the relay bar causing a contusion to right foot.

Over-exertion in pulling or pushing objects

Employee was recovering belt structure when employee felt pain in employee's back.

Struck by flying object

EE was replacing a bearing on a shaft. Later determined that metal sliver entered eye. Complained of burning in eyes. Refused to flush eyes and continued working. Treated eyes with Visine at home. Returned to work on 24th complained about left eye. Flushed eyes. At 1:30 pm sought medical attention at the ER. A metal sliver was removed. EE returned to work on 4/25/18

Struck against stationary object

A worker was descending on the man basket and at the same time another worker was rolling up an extension cord and wandered under the basket. EE stood up and hit EE's head on the basket.

Employee was removing curtain from shuttle car reel and felt pain in lower back.

Struck by flying object

Miner was walking on roadway at cooler, felt something go in right eye, was wearing all required PPE including safety glasses, had no issues with eye rest of work day, eye became irritated during the night, reported incident on 4/24/2018, sent to eye specialist, metal flake removed from right eye, written out of work for 4/24/2018 and 4/25/2018. Full duty 4/26/2018

Climbing onto a D9R tractor using 3 points of contact when ee pulled up ee felt a sharp pain in the stomach area.

Employee was traveling through a 30 inch man door going from the belt entry over to the track (intake) entry. As the employee was going through the door ee noticed a concrete block in the walkway. The employee extended left leg to avoid the block and when ee went to stand up ee twisted left leg and felt a ""pop"" in left knee.

A fully trained miner was center bolting when a rock fell from the roof striking the miner's lower back. The injury was not the result of any compliance issues nor were any protective guards involved.

Caught in, under or between a moving and a stationary object

Employee was placing a crib block under a prefab Track switch when the switch fell and pinched the right middle finger between the crib block and supply car causing a laceration.

Over-exertion in pulling or pushing objects

Employee was removing a hydraulic hose that had been disconnected on both ends and the clamps were removed from a 785 Caterpillar rock truck. As employee pulled they felt a pain in the upper back. Hose measured 36"" long X 1.5"" diameter.

The employee was cleaning a chute clogged with wet sand when the wrench ee was holding shifted suddenly. The wrench crushed ee's left thumb causing the bone to break between the two thumb joints.

Over-exertion in pulling or pushing objects

The employee claims that EE felt a sharp pain in EE's wrist while removing a bolt with a ratchet. EE recently returned to work from a non-work related injury on 4/9/18. An independent medical evaluation (IME) on 5/1/2018 determined that at the time of the incident of 4/20/2018, there was no new injury to the right wrist. Ramaco Resources, LLC is disputing this accident.

Employee was sitting in haul truck waiting for Trac hoe to load with boulders, when a boulder was dropped on tail end of truck causing the bed to raise and then slam down rocking the cab hurting employee back

A fully trained miner was setting the ATRS jack on a center bolter when a piece of rock fell from the roof striking EE's left hand. The injury was not the result of any compliance issues nor were any protective guards involved.

contract labor engaged in cleaning activity...windy day...got foreign material in eyes...rubbed them repeatedly in disregard to both advice and instruction...washed out with eye specific solution...EE went to WRMC ER later that evening...ER staff flushed eyes...prescribed an antibiotic as a precaution...released EE to return to work with no restrictions 04/21/2018

Employee was lifting metal screens, while bending over to lift a second screen employee felt pain and soreness in lower back area. Employee was taken to local Urgent Care for observation and diagnosed with a muscle strain.

EE was pulling rope through 8"" pipe which was attached to a 5"" hose. The end of the pipe 4.5' from edge. EE was near the face of a 15' drop. EE should've been tied off to prevent falling. EE wasn't. Rope broke & EE fell backwards & landed on feet & hurt left foot. EE had bruised hand w/minor cuts. Brought EE to ER, x-rays determined fractured heel. EE was wearing hard hat.

EE was operating a diesel getman on the supply road. EE encountered a loose concrete block on the blind side, which caused EE to be bounced in the seat. As EE bounced, they felt pain in their back. EE was given a full release to return to work without restrictions. On 05-08-18, EE had an appointment and the MRI revealed two herniated discs, EE was placed off work at this time.

Electrician was troubleshooting the limit switch circuit on #8 control sled on #20 dragline pumping system. Electrician entered the main disconnect cabinet to trace the wiring for the circuit. While standing on 4 ft step ladder, electrician reached toward back of cabinet to twist the label on the wiring for the limit switch to read it & experienced electrical shock.

Associate was attempting to dispose of an air filter, from a 988 FE Loader, when ee went to lift it into the dumpster ee heard shoulder pop. Upon examination from a doctor it was determined that the injury was a torn rotator cuff requiring surgery to repair.

While walking with a tool bucket on a concrete floor in the mill building, employee slipped and fell. Employee continued to experience discomfort so was evaluated by a physician on days off and received a positive diagnosis for a shoulder injury on 5/9/18.

EE was attempting to straighten 12"" U Bolt using a frame press with bottle jack attachment. EE turned U bolt on side to compress/reform bolt with a 4x4x2 piece of wood as a brace. When EE released bottle jack the stored energy caused the 4x4x2 piece wood to eject from the press, striking the EE in the face.EE was sent to emergency room for stitches.

Miner was cleaning out shaker screen house at the crusher when left arm was bruised. Material build under the screen housing and the material shifted and caught left arm.

Employee stated ee may have hurt shoulder pulling on a basket the shift before off days. The basket weighs approx. 23 pounds and is placed in the top of a tanker truck to catch oversized material. Employee was off 7 days for scheduled time off and did not report the incident until the day ee was scheduled to return.

We were contracted to change tires on a 844 John Deere loader. In the process of jacking the equipment up - the blocking and jack failed causing the loader to shift downward and the employee was struck by the loader frame.

While lowering the door to the mine floor EE caught left middle finger between a ""D"" ring on the door and a crib that was on the mine floor being used as blocking material. Suffered a fracture to the tip of left middle finger that required surgery.

The EE was lifting on a belt rail on the 001 section #4 belt to install belt structure and pulled a muscle in lower back. The accident was reported on the 4/20/2018.The EE started missing on the 4/23/2018.

Employee was bending over to pick up a piece of scrap metal and felt pain in the abdomen. Employee went to the doctor and was diagnosed with an umbilical hernia. Surgery is scheduled for 04/20/2018.

Employee was riding to workplace in rail man trip when EE said it hit a bump injuring EE's lower back. EE completed the shift and went to the doctor the next day. EE was taken off from work for 3 days for a pulled muscle. After the 3 days EE went back to the doctor and was taken off until 04/25/2018 and EE returned to work on that day.

IE stepped off step stool to the ground, stool slipped causing IE to fall to the ground injuring left shoulder. IE examined by the Coalition on 4/16/18 and referred to orthopedic specialist. MRI results were read 6/12/18 and indicated torn rotator cuff. Surgery was performed 6/18/18 to repair injured rotator cuff. IE worked normal schedule through 6/17/18.

Employee stated was downstacking structure on rail flatcar when another structure rolled down & struck right hand causing a large knot on the back of hand. EE & co-worker were working on each side of the car. 48' structure fell about 12'18' hitting EE in right hand which was holding another piece of structure.

EE was drilling holes in a dragline swing case seal so it could be removed. In the process,felt something in left eye and tried to wash it out. EE was transported to the ER. Next day EE was seen by a eye specialist to remove a small piece of metal from the eye, a prescription was given for pain/eye drops and EE was instructed not to return to work for at least 24 hrs.

Employee was in the process of changing a tie rod end on a shuttle car. The rod end was seized so EE put right arm against frame of shuttle car to increase force on wrench. While doing so EE felt a pop and instant pain in right shoulder. This employee returned to work the next day and recently went back to doctor and was placed off work for surgery as of 5/18/2018.

IE was carrying the hog panel from the hog panel rack on the roof bolter to the ATRS for the next row when the hog panel they were carrying got caught on the previously installed hog panel. After a MRI and a follow up Dr's visit on 4/25/18, the IE was diagnosed with a rotator cuff tear which required surgery on 4/26/18.

The employee was performing a workplace exam at the wash plant. When the employee stepped up and walked on a concrete platform for a piece of equipment, employee's foot slid out and employee almost did the splits. Did not become a lost time accident until 4/12/2018.

On 4/9/18 a third party trucking company driver was exiting cab at the Sweet S&G facility, when the driver's foot slipped on some built up ice on the truck step, causing driver to fall to the ground and injure ribs. It was later determined that the driver had broke a couple ribs from the fall.

Employee immediately stopped performing the task. Employee refused medical treatment at the time; however went to the ER later that night where they were diagnosed with a strain and given 1 day off.

Employee states employee bumped hard hat on a down pipe at approximately 7:00 pm. It did not knock employee down, knock off employee's helmet or cause employee any pain. Approximately 5 hrs later, employee was complaining of a headache & nausea & then briefly passed out so an ambulance was called. Employee was treated for concussion like symptoms and released from hospital.

Employee had fixed clear well pipe that was 11'-12' on platform. Employee was traveling back to the concrete floor when employee fell.

Dozer operator parked dozer on the stockpile at the end of the shift where there was a slope. It snowed the evening before and there was snow on the stockpile. The operator walked down the slope to the man van at the end of the shift and slipped on the stockpile.

Employee is a Saw Operator at the Chilton Quarry indoor within the Chilton Manufacturing Facility. Employee lifted a thirty pound stone from the Saw. Crew was using Catepiller Pin Pusher tool to install pin in pan feeder drive chain. Injured EE used a 10x1 inch bolt as a spacer to extend the length of the ram to finish driving the pin. As pressure was applied the bolt slipped and ejected striking the EE in the hardhat and forehead resulting in a skull fracture.

The employee had just gotten finished changing screens on the JCI screen. As ee was rushing to get back to the MCC to get the plant started back up, ee missed the second from the top step and fell approximately 10' to the ground, landing on left foot. The employee complained of pain in ankle. No fracture or deformity observed. Became a loss work injury on 4/19/2018.

EE moved to find remote, doing this a track of the stacker ran over EE's arch & toes on right foot causing fractures.

Mine Utility Operator was mounting a CAT 320-E rock breaker when right ankle turned on the step causing Operator to land on the ground with right ankle. This caused a Bimalleolar fracture to right ankle.

EE was loading glue on bolter when EE hip popped out of place and back into place pinching nerve. EE was released to full duty following medical examination on 4.10.18. A follow up appointment on 5.1.18 resulted in EE placed on restricted duty at that time.

When mechanic lowered wrench the bolt dropped down and hit EE's lip causing a split on the inside. One stitch was required as a result of the incident.

A mechanic was fixing a leak on the steering hose of a Komatsu 830e Haul Truck. A second employee shut the truck down so the pressure would drop off. The mechanic turned off the master switch and locked out the switch before commencing work. The pressure did not completely bleed off the hydraulic hose. When EE loosened the fitting, the hose broke free and struck EE.

Contact with electrical current

Employee was bolting in the #9 entry with EE's hands on the roof bolter reaching for a piece of drill steel when EE felt that EE had been shocked. This occurred around the same time that the electricians were troubleshooting the miner cable with the fault wizard. EE was seen by an ambulance service and both agencies were notified.

Employee was cutting steel bands from rod bundles when one of the bands sprung loose and contacted EE's lower leg. The result was a puncture wound that required sutures.

EE was attempting to secure a second clamp for a wire mount when the drill bit gripped the strap causing it to strike EE in the face and upper lip. EE received stitches on the lip and was released to work with no restrictions

EE was installing tromell on #3 Mill. EE was attempting to tighten the bolts w/ a spline drive impact. EE squeezed the trigger, the impact unthreaded the nut. EE's left hand was supporting the weight by the socket. The nut unthreaded the impact dropped & EE's right pinky finger was struck between the socket & the frame of the tromell. EE sustained a fracture to tip of finger.

Struck against a moving object

While removing overburden, the excavator slipped on wet black dirt down bench approximately 8 to 10 feet coming to rest on it's side. The operator was wearing EE's seatbelt and exited the equipment on EE's own. Emergency responders were summoned to the site and the operator was transported for medical review. The equipment was inspected by a vendor mechanic, no repairs needed.

While closing set of rear compartment doors on Fuel/Lube Truck, gust of wind blew door abruptly causing left pinky to be smashed into other closed door. Individual refused to go see a doctor that day, but on Monday April 9th, requested to go get looked at by doctor. X-Rays indicated dislocated pinky tip and doctor put back in place.

Employee was moving over the spray pipes on top deck of screen. When EE straightened up a ""zip tie"" that was holding a temporary light string in place went between EE's hard hat and safety glasses poking EE in the left eye. Prescription given for drops.

Mechanic was installing a pin during a new molebord installation on 78 dozer. When the pin fell into place, the weight of the pin pinched the employee's right pinky finger between the flat retainer plate and molebord ear, resulting in a laceration.

Struck by flying object

Employee was using a sledgehammer to change out a planetary on a continuous miner. While striking the planetary, a sliver of metal struck the employee in the left thigh. The sliver embedded in EE's left thigh. The employee finished the shift and did not miss any time. The employee, then had the metal sliver surgically removed on 09/06/2018. No work shifts were missed.

Employee was sanding the walkway due to slippery conditions on the walkway. EE was using a bucket and slowly dumping the sand in front of them when their feet slipped and they went down onto their elbow. Diagnosis for shoulder injury confirmed on July 26th.

Fall from machine

At the end of the day went to get out of truck and foot missed the step and fell all the way to the ground and landed on back.

The drill crew was in the process of anchoring the core drill, the binder was under pressure when it accidentally released striking the drill helper in the right side of the face.

Employee was getting metal out and tripped and fell on a belt roller. This resulted in a laceration requiring 11 stitches to EE's left leg.

After performing maintenance on a CAT 775D haul the employee was cleaning EE's tools when EE noticed that something was in EE's eye. After more than one attempt at flushing the eye the employee was seen by a physician who then prescribed antibiotic eye drops as a scratch on the cornea of the eye was found.

Employee was dragging a piece of Tensar mesh out of the face when left hand snagged an insulated cable hook. The hook punctured through the skin between index finger and thumb on left hand. Employee receive a tetanus shot and a prescription for an antibiotic and was released to return to work with no restrictions.

Contracting employee was cutting belt with razor knife, the knife slipped and contacted upper leg requiring stitches. Cause of this incident was body positioning and paying attention. Compliance with company rules a factor, mining/equip, PPE not a factor. Miner proficiency may be a factor.

Employee was grinding on a water tank when debris was blown into their eye by the wind. Accident date is 4/5/18. Treatment was First Aid. Injury became reportable on 4/6/18 when employee returned to clinic and received prescription antibiotics. Report was filed on 7/5/18 when prescription was confirmed. Employer was notified on 4/6/18 of prescription.
