An employee found this roof fall while travelling airways on the morning of 2-28-18 this area had been noticed beforehand and cribs and timbers where set in this area and route of travel was moved to another entry once the fall was discovered the area was dangered off and 4 breaker timbers where set the fall is apprx.40 feet in length 5 ft thick and covers the entire entry

Abandoned 4600 ""G"" entry had a unconfirmed panel collapse - pushing the water out of the old workings; water went down No. 3 Shaft and Galena Shaft - concern over damage to No. 3 Shaft. MSHA on site March 9th to look at entry
Unintentional roof fall was discovered on 02/28/2018 on the Bobcat Bleeder panel in the number one entry starting at just in by crosscut 11 and extends in by the number 12 crosscut
A non-injury ignition occurred at this mine in the face of the #7 heading. After investigation, it was determined that the machine had 16 dull or worn bits out of the 71 available. Due to the worn bits coming into contact with the sandstone intrusion in the face of the #7 heading, it is believed to have resulted in an ignition.
Non injury roof fall was discovered 2/26/18 at 6am on the return side #4 entry spad# 1 break inby 1814. Cribbed area off, did not impede travel &/or hamper ventilation. Approximately 8' high, 20' wide x 20' long.
An anonymous caller informed MSHA there was a fire at stockpile #4. There were no flames in the stockpile but steam and smoke were present from the dozer moving the coal to cool it down.
The BMX slope hoist was out of service due to a bad brake sensor. The brake sensor was replaced and the hoist returned to service.
The Chestnut Ridge Escape capsule was out of service for a extended period of time. The chain for the cable handler was found to be damaged. The cause of the damage is unknown. The damage was found during normal inspection.
The Rope Ripper Tripped on the Mine Elevator causing elevator to stop operating. There was an employee on the elevator at the time of the occurrence. Employee was stuck on elevator from 8:47 p.m. to 9:50 p.m. for approximately 1 hour and 3 minutes (63 Minutes).
An employee was traveling from the 1st floor to the 2nd floor in an elevator when it stopped 6-8' up. The elevator lowered itself to the first floor and it suddenly dropped 6-8"". The outer door was unable to open until the employee manually released the mechanics allowing it to open.
The elevator at Patterson Creek portal had a damaged bottom landing door switch. The switch was replaced and the elevator returned to normal service.
The mine experienced a possible face ignition on the 2 Left Section MMU 002. The ignition occurred at the face of the #6 entry approximately 103 feet inby spad number K143.
Slip in overlying rock pulled the overburden above the extent of the cable bolts.
Some hot coal started smoking in the high wall face. There were never any employees in any danger.
Tracking system was down due to a grounded reader.
A roof fall occurred in HG-6 MMU 003-0 in 2 entry XC 119
The cable on the slope hoist was found to have two wires broke on one strand of the hoist rope when it was inspected. The hoist was taken out of service and the hoist rope replaced. The hoist was back in service at 10:30 pm of the same day.
The CO2 system discharged on the dust collector. We called the fire department to assist. No open flame was seen but the internal temperatures caused the internal bags to start smoldering. There were no exposure to employees.
Power kicked on the surface causing power to kick on the hoist car which deployed the emergency brakes on the hoist. Power was restored and the hoist car reset at 11:07 AM.
As the foreman on #3 section was making faces, EE encountered that the top had fallen in a down cut in the #1 entry pulling bolts in the supported part of the mine roof. Both agencies were notified. No one was injured or trapped.
A roof fall was discovered at approximately 8:15 am on the morning of February 15, 2018 in the north main return # one entry between cross cuts 52 and 53. The roof fall measured approximately 50 long by 20 feet wide and approximately 5 to 6 feet in height.
Production hoist stopped working. A Non-Emergency Evacuation was initiated. Troubleshooting found that 2 auxiliary contactors needed to be changed out. The production hoist system was tested and the crew was allowed to go back underground.
Contractor on site caused an unplanned fire in the Ametek building of Kiln #1.
An unintentional roof fall occurred in the E-29 longwall section in 1-2 cross cut at 18 wall. The fall was approximately 20' high x 16' wide x 36' long. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the failure. The fall was just inby the longwall face at the time the fall occurred.
An unplanned roof fall was discovered in the Main North #5 ""idle entry"" between belt and track at 146.5 to 147.5 blocks. The area is approximately 12-14' high, 16' wide and 90' long. The fall was most likely caused by gradual deterioration of the intersection. This fall also did not disturb any ventilation, escapeways or travel ways.
I received a call from security had notified the mantrip bus was on fire. The security guard had stopped near the bus to eat lunch when ee noticed flames come out from under the hood. The job was idle no and employees were injured. This was an inactive part of the job where employees parked.
A roof fall occurred in the 1D3 Panel at cross cut 23 in #5 entry. The dimensions of the fall measured approximately 18' wide, 40' in length, and 10' in height.
The production hoist stopped working. Troubleshooting of the auxiliary contactors began. A non-emergency evacuation of the underground crew began but was halted as the auxiliary contactor was repaired.
Due to a power glitch two fuses were blew causing loss of power to the service hoist. No one was injured or trapped. All persons were evacuated and both agencies were notified.
A roof fall occurred, right return @ break 69 @ survey station #2009. Fall approx 5 ft high, 50 ft long. No injuries.
A roof fall was discovered at apx. 7:00am on the 3 south mains panel, 7c/c, #6 entry between spad #4128 & #4179. The fall measured apx. 74 in length, 20 in width and 2 to 8 in thickness. There were no injuries or interruption in ventilation. The area was dangered off, the HV cable was re-routed and timbers were set preventing access to all approaches of the area.
We had a roof fall in the Blue Rapids mine that was discovered this morning, 2/7/2018, during routine inspections. The exact time of fall is unknown but the area was observed yesterday afternoon too and the fall had not occurred then. No injuries occurred.
A non-injury roof fall occurred in the #6 intake entry between crosscuts 37 and 38. The fall measured approximately 40 feet long, 19 feet wide and 4 feet thick.
Elevator was down due to an electrical problem.
A 289D Cat Skid steer belonging to Gerke excavating was working on property owned by Hi Crush Augusta when it suffered a mechanical failure that resulted in said piece of equipment catching on fire. An apparent hydraulic line failure allowed oil to come in contact with the exhaust and/or turbo of the skid steer, causing a fire to start.
A guide rope interfered with the use of the 16 ft. shaft operation. One of the guide ropes outer layer broke down. The mine was evacuated and repairs were made to remove and change the guide rope.
A non-injury roof fall occurred in the #5 Heading while retreat mining was being conducted inby the 7th Crosscut of the 3 left Panel. Fall pulled roof bolts and caught the continuous miner in the 4th lift of the entry.
The crabapple portal elevator was not operational due to a faulty door limit switch.
An unintentional roof fall occurred in the E-29 Longwall section in the #2 track entry at 19 wall intersection. The fall measured approximately 20' high x 16' wide x 16' wide. The immediate roof consisted on laminated shale type material plus adverse geological conditions contributed to the failure. There were no injuries as a result of this incident.
The bottom door latch switch froze and interfered with the use of the elevator for over 30 minutes. The switch was replaced allowing the elevator to resume normal operations.
Two production employees were attempting to empty the pre-hopper of the fine coal bin. When they opened the gate, nothing came out. When they closed the gate, the employees felt a percussion, heard a bang and the room filled with coal dust. The employees exited the room uninjured.
Employee was electrocuted by transformer power.
The victim was servicing/working on the C/M around the scrubber motor area when a piece of the coal rib measuring approx. 3'L x 2'H x 15""T rolled out crushing the victim against the machine.
EE was working on a rock duster machine. The auger had become locked up. EE attempted to free the auger with a screwdriver but the screwdriver kept slipping. The EE thinking the machine was off tried to turn the motor by hand. The motor started and pulled EE left middle finger between the belt and motor pulley. The EE has returned on modified duty.
An operator was standing on the tracks of a bulldozer, when the operator inside the cab, accidentally hit the travel lever. The operator had foot pinched between the tracks and the dozer frame. EE broke 3 toes on left foot. One was removed at the hospital Friday night. Expected to make full recovery.
Two miners were replacing the drive belts on feeder when one of the miners got hand pinched between the belt and pulley, resulting in the tip of finger being amputated.
Employee was helping install a tram motor, as they were lowering the motor into place the chain slipped pinching EE's middle left finger between the frame and motor causing partial amputation to tip of finger requiring surgery.
Employee was struck on the right hand by a small piece of gob. The injury resulted in a fractured right middle finger and a partially amputated index finger.
Injured employee was standing at the headgate of the F-27 Longwall section when the decking at the HG #5 special pan caught the toes of their left foot between the decking and the mine floor causing an injury to the toes on the left foot that required surgery.
Feeder pan excessively leaked while running causing a buildup of material, pan roller were never guarded. Operator reached around and in to clean material without blocking motion. Roller and pan grabbed glove pulling fingers in, right hand pointer and ring finger were badly bruised, middle finger amputated 2/3 below first knuckle.
Injured stepped out of a motor to throw a switch and stepped on a one hole board and sprained left ankle.
We were pulling the hole and EE was stacking the auger back on the sled and when EE stacked the top one it didn't get seated far enough back in the cradle and rolled off the top and landed on EE's right foot.
Employee stated ee operating a roof bolting machine on the unit 1 section. In the process of operating the machine two rocks fell from the mine roof and struck the employee in the back, shoulder , and elbow.
Employee was walking on east track when they tripped over rope and fell landing on the left side of their head and shoulder hitting it on a rail causing injury to the neck. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND HAS BEEN TAKEN OFF ON 5/15/18 DUE TO SURGERY ON THE NECK.
Employee was replacing an ATRS jack on a continuous miner. EE was using a chain hoist to position the replacement jack into position. The chain slipped causing the replacement jack to slip and pivot striking the employee's right hand causing a laceration to palm. Employee didn't violate any rules or regulations and has adequate training and experience. Was wearing required PPE.
EE was helping another employee change screens in one of the rotex screeners. As the two employees lifted and slid the bottom deck screen in place, EE felt a sharp pain across EE's entire back.
A piece of rock 6x6x1 fell from the roof, striking the employee on the top of EE's hard hat. Incident happened on 2-27-2018, employee worked until 4-24-2018.
Employee stated EE was operating a shuttle car when the shuttle car struck a small rut causing the employee to jar EE's back resulting in a sprain/strain injury and lost work days.
Injured was retrieving ventilation tubes from #2 entry of 5E (inby last open), standing with tubes on left shoulder, went up on tip toes to unhook hold up chain and felt pain in abdomen, allegedly resulting in an abdominal strain that irritated 2 pre-existing (non-work related) hernia's. No witnesses. Incident is still under review.
Contract IE was unloading cribs from a scoop bucket. IE was reaching into the bucket to retrieve from a smaller pile when another employee cut the bands on a second pallet of cribs. They spilled over contacting the IE's right hand. Mach was made of aware of this accident 03/05/2018 via Compliance Staffing Agency.
EE was tying roof mesh to existing roof supports. EE was standing on a ladder in a x-cut by the haulageway. The piece of mesh EE was hanging hung down on one edge and was struck by an on-coming truck which pushed the mesh into the ladder, tipping the ladder over and EE fell to the ground, landing on right arm.
Employee was walking to check a pump in the return when they slipped on a slate rock, while trying not to fall they twisted the left knee and heard a popping noise. Employee finished shift and returned to work with no restrictions for next schedule shift. On 6/1/2018 employee had knee surgery and began missing work at which time this incident became reportable.
Employee was smoothing out a piece of cut metal when a tiny shaving got into left eye. Employee was using a grinder with a buffer wheel. Employee was wearing safety glasses and gloves.
Allegedly laying under 14 belt pulling belt over bottom rollers to make splice and felt pain in chest and back. Originally turned in under comp for 2/15/18 incident report where EE fell down. Incident report for this event not completed until 3/1/18. Alleged incident is still under review.
Two workers were moving pit pumps. The flange's on the poly pipe had to be removed. The fourth bolt would not come off, and the pipe was being held up by a chain that was attached to the excavator. EE used a torch to remove the bolt. When it came off, the poly pipe struck EE in the forehead.
Employee stated was in the process of hanging fly pads between #4 and #5 Entry 1 bk outby last open line. EE stated went to tear down fly pads outby in #5 Entry, when EE turned to take a step after tearing the fly pad down, EE said EE stepped in a hole and turned ankle. EE stated that it was wet and muddy in this Entry. Employee was transported to RGH by ambulance.
Employee was installing a belt chair at 8-54 belt while pulling the belt chair they felt pop in left shoulder. Employee starting missing work 09/05/2018.
Employee was unplugging a set of scoop batteries when employee lost balance and hit lower back on the corner of another set of batteries resulting in employee breaking transverse process. The employee is currently off work and actual return date is unknown at this time.
A fully trained miner was walking between ventilation curtains when EE stepped on a piece of coal causing EE to roll EE's right ankle. The accident was not the result of any compliance issues nor were any protective guards involved.
As the diamond core drilling crew was adding a 10 foot PQ rod to the drill string, the drill helper had left hand pinched between the drill head and the 10 foot PQ rod.
EE was preshifting a Dodge pickup. EE attempted to lift the engine compartment hood with left hand and the hood wouldn't move. EE then jerked on the hood with left arm and the hood still wouldn't move. EE felt pain in the left arm. Overexertion/strain to left bicep tendon.
Employee stated EE was lifting a belt scraper when EE felt pain in EE's lower back and both legs resulting in sprain / strain injury and lost work days.
EE was coming down in bucket to get equipment, leveling cable broke and bucket went forward and dumped EE out. EE has 4 broken ribs, broke collarbone, broke EE's glasses and laceration near right eye.
Employee was assisting in the installation of track rails on the 4-E section. EE was pulling on a joint of rail with a drag bar and EE's left arm gave out. Employee has continued to work until 5/21/18 when EE began missing work due to this injury. Employee overexerted EE's left arm pulling too much weight. EE didn't violate any rules or regulations.
Employee was traveling to the crusher with a load of shot rock when EE failed to maintain control of the truck EE was operating. The truck traveled over a berm and struck a pillar in the underground mine. The employee was taken to a hospital for treatment.
EE was walking from the underground parking area to cage to hoist out. EE stumbled and fell to the ground, twisting right knee.
Employee was bending over corner of bundle of wire mesh and cutting metal band. When the band broke loose it hit EE, knocking hat and safety glasses off. This resulted in a cut to EE's eyebrow and eyeball.
Employee was working off side of operator preparing to load roof bolter, as the partner backed up they walked too close allowing employee's right foot to be run over by front tire of bolter. Employee was taken to Doctor the day of accident and returned 8-23-2018 without restrictions and during follow Doctor visit 2-27-2018 was given restrictions and became lost time 2-27-2018.
Employee stated while installing a left hand turbo on an off road truck they felt a sharp pain in right shoulder didn't see a physician until 3/27 and was placed on light duty - non available off work as of 3/28/18
Employee was using a hand grinder to remove a separator roller in a conveyor belt take-up. During the process EE struck self with the grinder that was equipped with a 4 ½"" cut off wheel on the top of left hand between thumb and index finger. The injury required surgery to repair the extensor tendon in the left thumb and index finger.
This report is prepared to comply the requirements of MHSA. 2/22/18, employee alleges that when ee performed the cleaning activities using the skip loader in the transporte#3, hurts ee back after hitting a bean when leaving the area. 2/26/18 employee was referred to Argos Medical provider which referred to workers compensation.
Employee was discarding used oil in the oil shed outside the surface shop. EE opened the doors to the shed, while entering the doorway, the wind blow the left door into EE and contacted EE's left lower hip/back. Seen by onsite medic referred to next level of care. Given restrictions.
While cleaning the right side of a cut (taking the pie out of #1 ent.) the rib rolled out striking the operator in the back.
Employee walked out of building into the rain and slipped on wet and muddy pavement and broke lower leg
While unloading roof control supplies in a scoop bucket, injured slipped and fell twisting left knee and bumping head. While at hospital, employee had a severe allergic reaction to the contrast dye injected to perform a CT scan and was hospitalized.
Employee was headed to clock out, didn't follow the treated walking path and slipped on ice.
Employee was using brake cleaner to clean a fly wheel housing to install a bearing. Employee began to heat the clean part with a torch and the cleaner fumes in the housing caused an explosion. Employee was taken to the Emergency Room and treated for First Degree Burns on arms, neck and face.
EE was cranking the bench vice when EE felt a sharp pain in the center of EE's back.
Employee was lifting on ""Leveling Jack Pad"" when employee felt something pull in shoulder
Employee slipped on the ice walking to a crew van causing pain in EE's right knee. The employee had a non work related surgery on this knee in Nov 2017, an MRI was performed on 3/6/18 which resulted in knee surgery on 3/20/18 with a diagnosis of a recurrent symptomatic medial meniscus tear. The first day the employee missed work was on 3/19/18
Employee received a contusion to the head when EE lost balance and fell from the second step of a 6 ft. step ladder at the F-Portal office building.
Injured was loading miner and feeder cable onto a flat car by hand with two other employees at the end of the track in the 4 East section. EE felt pain in EE's upper and lower back. EE was diagnosed with strained thoracic and lumbar ligaments.
Employee parked D10 dozer & was dismounting when a gust of wind pushed the door & employee onto the track crawler & down to tilt jack. Cause of injury no 3 point contact while climbing down. Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. Started missing work 2/27/18. Work was idle from 2/21/18 till 2/27/18.
Improper load positioning on work boat crane causing the boat to list to one side resulting in boat capsizing.
EMPLOYEE WAS REMOVING A BAGGING CHAIR FROM AIR FLOAT BAGGING WHEN EE EXPERIENCED PAIN THAT AGGRAVATED AN ALREADY EXISTING BACK PAIN ISSUE.
EE got a piece of concrete in EE's left eye while working in a man hole. Concrete flew onto EE's safety glasses and into EE's left eye causing 2 abrasions. EE was taken to the ER.
It was raining and the employee had an umbrella in hand. Employee was walking around the corner of the security post in order to enter the building. Employee caught the toe of employee's boot and fell forward landing on left arm on the pavement. Employee fractured the radius bone in left arm. Employee's first day away from work was 2/20/2018.
EE was stepping out of a mantrip, as EE got out EE stepped on a piece of coal twisting right ankle causing pain and swelling.
Employee was working along the Section E beltline. Employee was dragging water/pipe line and tripped over other pipe line in the work area.
Despite having plowed and applied scoria in the morning, a mechanic slipped while doing a pre-shift inspection on a tele-handler forklift and fell. Initially EE said was fine but on 2/22 EE said that EE wanted to see a doctor because shoulder was hurting.
Employee was in the panline on the F-27 Longwall face hooking the panline up to shield when a rock slid out of the face striking employee on the left lower leg causing right lower leg to strike the eiko track. On 2/28/2018 employee was diagnosed with a non-displaced fracture of the fibula & a partial tear of the lateral collateral ligament in right leg and began missing work.
As employee was cleaning the snow and ice off the platform scales at the Bakersville scale house. The shovel hit a hard ice spot abruptly stopping the shovel and a snap noise in the employee's right wrist. Causing some discomfort.
Following milling and scaling, the employee was working at the face to prepare a heading for a shot. While performing the work, a rock fell from the face, striking the employee in the leg. First aid was provided at the scene and the employee was then transported to the local hospital for further treatment.
Employee was lifting structure at the face when ee felt pain in lower right abdomen.
Employee picked up a hydraulic jack to put back on the locomotive and felt a pop in the left shoulder. Employee worked till 2/27/2018 when EE was taken off work.
Injured employee was flagging an operator to lift a piece of poly pipe, when the forklift raised the pipe ice began sliding out of the pipe striking the injured employee.
Injured was stepping out of EE's D10 Dozer at the end of EE's scheduled shift. As EE stepped out EE lost EE's balance, EE's foot slid between the track and dozer frame, EE fell backwards fracturing EE's left leg below the knee.
Employee was walking on section when ee slipped and felt a sharp pain in left knee. Employee sought medical treatment and was given a full release. On 5/2/2018 Employee began missing work due to knee surgery at which time this became a reportable incident.
*****Alleged***** TD-44 operator reported dozing in barn when equipment rolled over a chunk that resulted in jarring of back. Operator was seen by site responder, and then brought to health care facility for further evaluation. Given time off and restrictions.
Loading a shot, bent over to pick up the loading hose, back went out.
(REPORTABLE ONLY DUE TO SUTURES) - While loading two inch water line, Employee caught right index finger between the end of the pipe and the forks on the Lo trac requiring ee to receive about eleven (11) stitches.
Employee was drilling the mine roof and was inserting another piece of drill steel, pushed it into the top and strained shoulder. Employee started missing work 3/6/2018.
EE was exiting the Room 100 trailer office in the rain. EE was descending the stairs when EE's rubber boot slipped on the stairs and EE fell down the stairs. EE sustained a slight fracture in EE's fibula.
A rock slipped from the excavator bucket onto another rock, sending a fractured piece of rock flying into the cab of the excavator. Fractured piece of rock struck the employee in the right eye area causing a ruptured right eye.
Employee was center bolting in the F-29 section. While removing drill steels from the roof ee pinched left 5th (pinky) finger between the connection point of the bottom two steels causing a laceration to side of finger that required 7 sutures to repair.
Excavator operator was loading haul truck operated. The truck was nearly loaded when a large sized rock rolled from excavator bucket and fell landing on the rear of the haul truck bed causing haul truck to rock backward jarring operator in EE's seat.
Employee stated ee had already installed 2 bolts in row and decided to install a spot bolt next to the rib. After installing the spot bolt, ee swung in and a rock 48"" x 32"" x 4-5"" fell out between bolts hitting ee in the back. EE went to Princeton Community Hosp. ER. X-ray indicated possible fracture of C-7 and ee was given time away from work and referred to specialist
Cable reel was engaged on roof bolter and when began taking up trailing cable slack the cable struck employee on the left leg causing muscle contusion.
Employee was helping to load rock dust in the scoop bucket with the use of a diesel forklift. The forklift had a bulk bag of rock dust on the forks & EE was cutting the bag to allow the dust to fall into scoop bucket. Employee positioned EE's right leg between the scoop bucket & the forklift tire. The forklift then pulled forward catching right leg between the two machines.
The employee was preparing to roof bolt the crosscut of #5 entry when a rock fell from the unsupported roof on top of the canopy of the roof bolter. The rock broke and cantilevered off the canopy. It landed vertical and when it fell to the side, the employee got caught between the rock and the tray of the roof bolter. This caused a non-displaced fracture to one rib.
Employee was doing maintenance work on Cat D11R dozer. EE slipped & fell on the cleated foot step causing a gash to left shin. Medical attention resulted in 5 stitches to close the wound.
Injured employee climbed on top of EE's parked shuttle car so EE could reach the mine roof to re-hang a communication antenna that had fell down. As EE was hanging the antenna EE slipped and fell landing on the side board of the shuttle car. Employee suffered a contusion to EE's groin area. Employee has proper training and was wearing required PPE.
Employee was shoveling and cleaning around the crusher when a rock fell and hit hand.
Employee was exiting vehicle and pinched left middle finger tip between the door and door jam of the cable truck breaking finger tip bone and requiring stitches.
Employee was installing a roof bolt in place of a loose pin, due to scaling rock. When rib and a section of the roof rolled off striking the employee from behind on left lower leg.
Outby crew was moving a roof bolter off of 002 section to the #2 mains, the crew had to re-hook the pull chain, employee stepped between the scoop and roof bolter when bolter rolled, pinning EE's left leg.
After first dipper was placed in haul truck, material rolled down from shovel bank, striking the filter housing, and jarring the driver. Driver initially refused treatment, but the next day prior to the shift, went to the ER and was taken off work until 2/16, a day off.
A fully trained miner was loading rail sections from a take up unit when the rails shifted on the car and a rail hit EE's right ankle. The accident was not the result of any compliance issues nor were any protective guards involved.
Employee was moving water pump when employee strained right shoulder. Employee did not start missing time from this injury until 03/12/2018.
Employee was walking down No. 4 entry to hang the miner cable. Employee tripped and fell hurting right hip, right knee and lower back. Employee missed two days of work.
Employee was walking around battery charger when ee tripped and as ee tried to catch self ee twisted injuring lower back.
Had been raining earlier that morning and ladder steps were wet.
EE was checking supplies on the walk-through bolter and as ee exited off of the bolter, ee slipped and fell to the mine floor, striking right shoulder against the floor. EE continued working without restrictions performing normal occupation until 03-05-2018. EE began missing time on 03-05-2018.
Employee was pulling a rock out of tail piece. A rock became fouled at the 4a Head Drive # 1 Tailpiece. Employee was pulling the rock out when EE Strained/sprained EE's right lower back. EE took self to the hospital to be checked. EE was taken off from work for 2 days. EE returned to work on 02/14/2018 with no restrictions.
Employee had left jackleg machine extended in the back and turned back to the machine to get more supplies when the machine fell contacting EE in the back of the hardhat. EE was seen by the site medic, referred to the ER, staples were required to close laceration and evaluated with a concussion, restrictions were issued.
Employee was in panline of the longwall face hanging recovery screen when EE twisted left knee. The employee continues to work but started physical therapy on 03/08/2018. This is now a re-portable NDL accident.
Employee was lifting lid on 20 ton diesel motor and felt pain in the lower back. Employee worked till 2/13/2018 when they went off work.
Technician was on on-call weekend and was unloading and placing barrels. The technician had to cross over a concrete transformer pad. There was a hole that was not covered and the area was poorly lit. As the technician turned around the technician missed footing and tripping on the edge of the hole and foot going into hole.
Employee was roofbolting, installing a bolt in roof with board and as employee was lifting up felt sharp pain in right shoulder causing injury to shoulder.
Employee stated EE was tramming EE's shuttle car from the C/M machine to the feeder on the Unit 1 Section. In the process of turning the corner at break #9 in the #6 intersection at Spad #3234 a rock fell out from the rib and struck the employee in the hand.
Employee was riding out of mine on mantrip. The mantrip hit a motor stopped on haulage track. Employee head hit Plexiglas of windshield and ee body was thrown forward.
Employee was assisting 2 others in rotating the drums of the CM when they felt pain in the lower back/hip. EE refused 1st aid, waited for a short time but then exited the mine to recuperate. EE traveled home but on 2/10/18 at 4:45 pm stated they were going to RGH ER to be checked. After examination, EE was give time away until 02/16/18 or sees a PCP. EE remains off work.
Injured employee was using a track wrench tightening track bolts when EE felt pain in lower back. Employee continued to work until 02/13/2018 when EE began missing due to this incident. Employee didn't violate any rules or regulations, has proper training and was wearing required PPE. No mining equipment was involved.
EMPLOYEE SUFFERED ABDOMINAL PAIN THAT MAY HAVE BEEN CAUSED BY STRAINING SELF. DOCTOR DIAGNOSED AS A HERNIA
EE haulage truck was transporting coal to typo tipple on haul road at east long mine when ee truck was struck in the side from behind by another ee truck driven. The other ee jumped from the truck making it a runaway because it was said it would not stop
Plant maintenance, slipped in the mud and twisted ankle. Went to hospital found ankle was broke. Did not receive this until this date 03/15/18.
Driver lost control, jumped from moving, loaded vehicle. Broke both feet.
While assisting with lowering a bucket elevator cover, employee's finger was pinched between the cover and the access ladder, resulting in a laceration requiring stitches.
Employee was shoveling material off a platform which is below the primary screen tower when a rock fell from above and struck employee on the side of EE's hard hat.
EE said ee was turning a light switch off on A-40 Volvo articulating truck and jammed hand on dash of A-40
Employee was unloading belt structure and pulled a muscle in their back.
While employee was installing a 5' roof bolt after drilling the hole, a piece of draw rock fell, hit the pan on the bolter, then fell and hit employee on the left foot. This resulted in a cut to the left small toe that required stitches.
A fully trained miner was working in the motor barn when EE stepped on a rock straining the arch of the right foot. The accident was not the result of any compliance issues nor were any protective guards involved.
Miner was locating an electric motor in warehouse. EE slipped and struck right knee on the shaft of a motor sitting on the floor. After several weeks of monitoring and evaluating, it was determined on 03-13-2017 that surgery was needed. Surgery was performed on 03-15-2018.
The employee drilled starter steel into the mine roof and dropped the boom. Employee's partner was trying to communicate with employee so employee covered the second drill steel in the chuck with hand to hear better. When employee did this, the first drill steel fell out of the mine roof onto the employee's hand causing a fracture.
IE bent down to pick up a battery jumper cable and felt a pop in IE's back. IE was transported out of the mine for medical treatment. IE has been diagnosed with an annular tear and disc herniation of L4-5.
Employee was using a pole ax to loosen track ties from the rail at the end of 9M track. The employee started to feel a pain in lower back while bending over and striking the track ties with the pole ax.
Miner was cleaning coal from a chute with a scraper when ee lacerated right hand
A contract employee was roof bolting. EE bent drill steel, and while trying to correct the problem the drill steel hit EE's left hand. EE suffered a contusion.
Employee fell getting out of truck in the parking lot before shift. An MRI of shoulder later revealed the need for surgical repair. EE started missing work on February 26. 2018.
The employee stated EE was attempting to make a belt splice when EE struck left middle finger with a hammer causing a laceration to the finger.
While fabricating starter sheets, standing on grating, the grating separated, causing employee to roll ankle. Employee was taken to the clinic and taken off work 1 week. On 2/15, employee returned to PCP, was given a boot, and taken off an additional week. Currently ONGOING.
IE and another employee were installing a 24"" ventilation tube regulator in 2-Left Face. IE's right thumb was sucked between the regulator and another tube when the regulator accidently fell shut. This shifted the ventilating pressure to a 1"" gap between tubes. The IE's right thumb was smashed between tubes.
On the morning of the injury, the weather was freezing rain, then turned to snow. The temperature was approx. 14-15 degrees. EE exited the dozer. Once ee was on the ground, took two steps and stepped on a 5' patch of ice which was covered with snow. EE's feet went out from under self. EE fell directly on right shoulder.
Employee had just turned the exhaust fans in the shop off, as they turned, the left foot caught a cable holding cap, causing them to fall. As they fell, they extended the left arm to catch themselves, and injured the left shoulder. Employee continued to work unrestricted until 04/24/2018, when employee was taken off work for surgical repair.

Employee was prying a pin out on a 14H Grader and right shoulder started hurting. EE started missing work on 2/7/18
2/6/2018, employee was walking on ground level walk way, a hose from a super sucker vacuum truck was lying across walkway. As employee attempted to step over hose EE tripped and fell. Employee injured right knee. Employee was able to continue to work regular work and shift at full time status. 7/31/2018 employee diagnosed with meniscus tear and received surgery.
Employee was getting out of EE's personal truck and when EE stepped onto the ground felt EE's hip pop. EE's truck is lifted and is very high off the ground. Employee was in awkward position and has had a previous hip surgery on that hip.
Employee was lifting an oxygen tank and strained abdomen. Employee did not go to the doctor until 7/9/18 and was diagnosed with a hernia.
Employee stepped in mud along the track and as EE stepped out of the mud, EE felt a pop in EE's right hip. It also did not involve job skills, proficiency, training, PPE or protective devices.
Employee was walking across the unit when they stepped in a hole twisting the right knee. Employee began missing work the next day due to the injury to the knee.
The employee bent over to move the miner cable and strained left knee.
Stepped on a rock and twisted ankle resulting in a sprained ankle.
Slipped on ice and fractured left fibula
Clear & calm. Operator placed lunch bag on cab platform, climbed ladder to cab platform, received lunch bag from in front of cab door, opened door & moved lunch bag into cab. Operator hyper extended right knee as EE was placing the lunch bag in the cab. The hyper extension of EE's knees caused the ham string muscle tear - strain.
Employee was walking from car in the employee parking lot and slipped on black ice. Employee broke right ankle.
The miner was using a pick ax to dig up buried SO cord and wire. The miner felt soreness in back after work. EE woke up the next day and felt more sore. Was diagnosed with a back strain. Lost time as of 2/7/18.
On 2/4/18 employee fell while getting on a scissor lift truck causing a contusion to EE's left hip, did not report the incident & on 2/5 EE was noticed limping by supervision & asked what happened. EE explained & was alright, on days off was not better so EE doctored on 2/9 not mentioning it was work related. 2/11 doctored again & was given restrictions making this reportable.
Wet Plant Attendant reconnected a 2"" rubber airline to a 2"" metal nipple. The airline was recharged and end of airline blew off nipple striking employee in mouth. Employee was taken to ER in Kermit, Texas where EE received 3 stitches in lip. Also has a broken front tooth. Clamps were not tighten enough to prevent blow off.
Employee was attempting to tighten bolts on a cutterhead module of a Highwall Miner when the wrench slipped and employee fell to the ground breaking right leg.
Employee was cleaning up around timber when it became dislodged and fell, striking employee's left hand (thumb). It was later determined that a bone in the employee's wrist was also fractured. Employee started missing on 03/07/18.
Employee was splitting granite block with feathers and wedges upon splitting the piece of stone employee was standing on shifted unexpectedly causing employee to loose balance falling forward onto adjacent piece of granite causing abrasions to both knees and right elbow.
Employee was watching trailing cable slack on the #2 roof bolter as it was being trammed. A scoop, 1 break outby, unexpectedly caught the roof bolter trailing cable pulling the trailing cable and entangling the injured's foot in a loop of the trailing cable slack. Resulted in a fractured tibia of the left ankle/foot area.
A fully trained miner was unloading belt structure when the miner dropped a bed rail on right foot. The injury was not the result of any compliance issues nor were any protective guards involved.
The employee's pants leg caught a piece if grating causing them to fall. The employee denied medical attention until 2/5/2018 at which time they were placed on restricted duty.
Employee was attempting to run a sample in the lab at Upper Gieseke. The disk that sits on top of a heavy ceramic bowl was situated incorrectly in the bowl. Employee tried to adjust disk by tapping bowl against ground. Bowl slipped and mashed employee's index and third finger on left hand, resulting in laceration of finger.
Employee tripped on stretchwrap and fell. Employee reached out with a hand to catch themselves and hurt a wrist. After a few days employee went to the doctor who determined it to be a fracture.
The employee was adjusting a bench and strained back.
Employee was removing a 5-6' diameter 3000 lb drive sheave on a horizontal impact crusher to prepare rotor for removal. Sheave was seized on shaft and popped off once it came loose.
Jumped off trailer that is approximately 18 inches off ground and twisted knee.
Employee was roof bolting. When EE attempted to straighten a bolt for installation EE felt a pain in left shoulder. EE continued to work until 03-19-2018. EE was taken off for shoulder strain.
A fully trained miner was shoveling a belt take up when ee fell backwards into a brace injuring back. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee standing beside 988F loader holding grease hose on fitting, co-worker moved bucket causing right forearm to be caught in pinch point.
Employee was walking from the upper shop to another location but traversed through ice that had not yet melted from the most recent storm. EE slipped and fell.
Another tech and myself started to disassemble the accumulator and failed to bleed off the nitrogen before starting disassembly. I was holding the ratchet on the end cap while ee ran the ratchet. When the end cap came to the end of its threads the pressure that was still in the accumulator blew the end cap into my hand.
Employee was walking and transitioned from a paved surface to an unpaved surface and stepped into a hole injuring lower leg.
Operator off loading stone off conveyor belt, lost grip on the stone and dropped it on foot above the steel toe boot. Operator expressed just sore then 2 days later went to Dr who diagnosed fracture on top of the foot.
In the process of lifting a sling the IE felt a ""pop"" in the stomach. During medical evaluation the IE was diagnosed with a hernia around the belly button. It has yet to be determined if surgery will be required. The IE was placed on light duty restrictions.
6 man crew was planting vegetation in 18"" of water. Worker was bit on left side by an alligator. Pre-job inspection showed no alligator presence. Spotter in place did not see gators until incident occurred. Employee taken to emergency room treated for injury on left hand, significant damage to thumb. Also cuts on torso under left arm and abrasions on back and stomach.
The miner was replacing the turn table on SCI's VSI crusher. When pushing the heavy table into place, EE sprained EE's knee.
EE was covering a pile of clay with a tarp when EE stepped onto what EE thought was solid ground. EE soon realized that the ground was giving way. EE fell and when EE was able to get up EE had extreme pain from EE's ribs.(It was raining at the time). EE notified EE's boss and boss took EE to the emergency room.
Employee had positioned side of the machine to install the next row of bolts. EE then extended the slide of the canopy out and attempted to raise the canopy higher. EE unknowingly left finger on the top side of the canopy which pressurized against a rock brow causing a fracture to right pinky finger.
Employee was pulling on tire chain. Employee was using a tee bar with a hook on the end to pull the chain in place. The employer was pulling hard and the hook slipped of the chain causing the employee to fall on back
Employee began to experience irritation in the right eye but did not recall a specific event to cause the irritation other than inadvertently contacting a pipe with a bar causing dust to fall from the pipe. During evaluation IE was given prescription medication, taken off work for 1 day and then placed on restrictions.
Injured was a passenger in a man-trip which struck a roof jack which was installed in concrete along the travel-way. The corner of the bumper caught the jack causing the injured to strike EE's knee against the dash of the mantrip.
While conducting a maintenance task on a screen, an employee was using a pry bar to lift grating to access the maintenance point and felt pain in back while lifting. The employee has missed work due to the injury, thus constituting MSHA reportability.
Weather was cold at 36 degrees. Crew was rebuilding crusher cone. Employee tripped and fell on un-leveled ground and hurt right shoulder.
While lifting the tongue of a Godwin Pump trailer, employee suffered a thoracic back strain. Employee was treated by a physician and taken out of work for a few days to recover.
Employee was attempting to walk through mud at grinding plant transfer building. The employee strained left knee when walking. This was initially reported as first aid on 02/23/2018, but was not placed on restricted duty until 03/09/2018.
While removing torque out of truck, ee reached over frame and started pulling wrench towards ee and felt and heard something pop in shoulder .
Employee stepped down approximately 14 inches from a trailer when left knee buckled.
Initial accident did not result in injury that was reportable. Only after several months the incident resulted in surgery. Shoulder did not improve and injured person sought medical attention.
Miner was using a 2 ton chain come-a-long to jack up a shaker and change a motor mount spring, when the come-a-long chain broke. The chain struck left elbow and the block (housing) struck right forearm and right leg.
EE felt pain in back during repetitive bending during the drilling process. EE went for medical treatment on 2.26.18 and was placed off work at that time.
Employee was clearing bridging wet Aluminum Trihydrate material from bucket elevator inlet chute with a shovel. The area was restricted access with barricade and signage. Shovel got caught to rotating buckets of elevator causing fingers to pinch between the shovel handle and inlet chute. EE received treatment from hospital ER for finger closed fracture and nail avulsion.
While adjusting crusher setting employee tripped over cable strung between crusher assembly and loader. Employee fell onto walkway causing contusion on left knee.
Employee requested medical treatment on 2/21/18 and was prescribed work restrictions by the occupational physician. The employee alleged that ee stepped on a rock, felt two pops in L-knee and felt pain. This is an alleged, non-witnessed questionable event.
Employee was replacing bottom metal screen deck which caused pressure to right wrist. Employee was on stomach between the top and bottom decks. Employee had an injury occur to the same wrist in past. When putting the deck into place employee said ee felt a sharp pain and lost feeling in hand.
While standing on the fuel tank, steam cleaning 769C Cat truck, employee stepped backwards and fell approx. four feet injuring left foot. Medical treatment resulted in modified work activity.
As employee was climbing the boarding ladder on to 59 haul truck they contacted their knee with the boarding ladder attachment. Employee later called the supervisor reporting that their knee was injured and their neck was sore.
On 2/16/2018 an employee was kneeling down and as EE stood up, they felt a pop in their left knee. On 3/15/2018, the employee was taken off work for surgery to repair a torn tendon in their knee.
The ground had patchy ice spots. Employee was walking from Bulldozer across parking lot to truck.
While installing an electric motor on equipment I.D. 351-BE-10, the employee was standing on the outer casing of the bucket elevator to set the motor in place. The employees were using a metal beam to hoist the motor in place. The beam slipped and dislodged from its resting location and onto the floor decking. The beam struck the employees lower leg area causing the injury.
Employee was moving a oil barrel by rolling it when EE felt instant pain in EE's right shoulder.
Contractor turned portable belt sander on as they turned the sander on its side, their finger became caught in between the rotating belt guide on the sander, creating an avulsion to left hand index finger and tip.
The injured employee was sprayed by pressurized hydraulic oil during the removal of a sensor. The release of the pressure caused EE to move rapidly and had struck the ground. Injuries were sustained to face and hand.
RIGHT KNEE BUMPED THE FIRST STEP OF THE LOADER WHEN CLIMBING UP TO START IT.
Employee was placing and pushing stone while operating the breaker when EE felt pain in back. Employee first reported this as a not work related back injury. Injury was determined to be work related on 03/20/2018.
At approximately 1308, an employee was assisting with removal of mast pins. The employee was holding a brass knockout for pin removal. While removing the final pin, the mast shifted and landed on the employee's right hand resulting in lacerations to the middle fingers. Employee was taken to the clinic and received 5 stitches.
2/13 EE was lifting a piece of stone from the pan on to the Thin Line machine when EE felt slight throbbing to lower left side and worked until shift end 3:30pm.
while using hydraulic torque wrench to tighten eccentric bolts on the primary crusher, the wrench came loose when pressurized. As employee attempted to hold/control the wrench, their right shoulder was extended causing a strained bicep.
Miner was placing tools and materials in the back of a maintenance truck when EE felt something in EE's boot. When EE took another step a nail protruded through EE's boot sole and into EE's right foot.
Employee was setting cribs when a piece of rock, measuring 19"" long by 14"" wide on one end tapering to a point on other end and 4"" thick, fell striking miner on left foot. The triangle shaped rock punctured the employee's outside left foot causing a laceration and fracture.
Employee stated they were picking up a 48 inch piece of belt structure on the surface. In the process of picking up the piece of structure they lost their footing in the mud, slipped, and felt a pain in the groin area.
Employee was carrying a 5 gallon diesel can about to enter a door to the mill. The employee set the can on the ground to go inspect a noise at another location. When they started to walk away they slipped on a patch of ice with the left foot. They fell on their right side injuring a shoulder.
EE missed step descending stairs at truck load out, as EE stumbled EE held onto handrail, straining the right shoulder. EE reported injury, but did not request medical attention for 2 weeks, until discomfort continued. Physician prescribed anti-inflammatory medication, and lifting restrictions.
It was lightly raining and an employee slipped on the top of the steps and fell down 4 steps. While trying to catch self, employee twisted back.
Working packaging bagged coal, employee attempted to clean spilled coal off stacking table w/ right hand. Top of right hand brushed against automatic bag sealing machine discharge area causing abrasion to back of right hand. Employee was taken to local Urgent Care 2/09/18 for observation and x-ray, and diagnosed with abrasion to right hand.
Bulk bag fell over while employee was placing strap onto forks of fork truck. Employee lost balance and fell forward. Employee later in the day noticed back was painful.
SNOWY CONDITIONS CAUSED SLIPPERY TERRAIN. EE FELL AND BROKE RIGHT ANKLE.
Emp was assisting to put CAT988 loader cutting edges back on the forks that slid off and onto the ground. Emp was using right hand to push one of the cutting edges to straighten it out so the forklift could pick it up and felt 3 pops in right elbow. This event became restricted duty on 2/12/2018.
Employee's back became stiff and sore after several shifts of replacing/repairing overhead lights around the facility. On February 6th the employee's back went into a spasm while ee was at lunch. Inflammation had developed causing low back pain and spasm.
Employee was preparing to perform maintenance on plant. Employee was pulling pin to access screen deck for screen repair, tray shifted and injured arm.
Employee was cutting loose a steel plate at the ABC reclaim hopper with a torch. As EE was cutting the plate loose, the plate became unstable and fell on the employee's left leg. The employee was performing maintenance on the hopper at the time of the incident.
Employee was loading rail cars with lime product. Employee had to release rail car brake by climbing rail car ladder to platform. After releasing the brake the employee started moving back towards ladder. During that process, employee fell to ground causing employees ankle to break.
An employee was attempting to manually raise the boarding ladder and felt pain in abdomen. On 3/1/2018, the employee received a confirmed diagnosis of a hernia.
While air arcing on feed cart, employee's clothes caught on fire.
Employee climbed up short vertical ladder to look at belts on elevator. When descending ee stepped off ladder to platform wrong and twisted ankle.
Clear weather, good lighting. Employee had finished changing a tire and was putting dismounted tire onto lift gate. Mis-stepped and fell from lift gate, landing on EE's back on the ground. Received a skin laceration, which received stitches at Tripler Arony Medical Center.
EE was tasked to shovel spillage from a conveyer, the following morning EE reported pain in EE's right bicep and shoulder. EE was seen by a physician and treated for muscle strain/tendonitis, and placed on restricted duty limiting lifting and use of EE's arm until cleared to return to full duty.
Mine RR ops employee was changing oxygen cylinder within a cabinet. Upon removing the cap ee noted ee needed to rotate the cylinder to line up the nozzle with the regulator. While turning the cylinder employee felt a pop in left bicep. Was diagnosed with a ruptured left distal bicep tendon.
Employee was offloading cribs from the de-energized Mule to shield toes on the longwall tear down face. While performing this task, EE was bending, twisting and not using proper lifting techniques. No equipment involved. It did not involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
EE was walking under the washer and right foot caught in grating causing ee to fall. EE struck right back against concrete pedestal base as ee fell. EE was treated at onsite clinic & returned to work. On 2/18 EE had back pain and went to ER for treatment. EE was given pain meds and instructed to return to work on 2/21 (two days off).
Backing down the 300 ton hopper ram in a FEL employee ran into the trailer of a truck, and hurt their back. EE went to the Doctor on 3/1/18 where they were put on restriction.
EE was helping install rub rail back on the miner. EE was holding the rail in place while another employee was striking the rail with a hammer. The rail jumped into place smashing the right ring finger between the rail and miner. Employee was treated and released from GVMC. EE was diagnosed with a fractured tip of the right ring finger and required 4 stitches around fingernail.
EE was helping install a log washer in BKS. During this activity EE was utilizing a 6' step ladder. EE unfolded the step ladder and placed it on what they thought was stable ground. Approximately 3 steps up the the ladder they either lost footing or the ladder became unstable causing them to fall to the ground.
Electrician was using retractable box knife to remove tape from motor lead. Electrician cut palm of right hand, requiring 9 stitches.
Rotex operator was performing a screen change on #3 rotex with department helper. While carrying a screen to 3 rotex, rotex operator dropped the screen & hit/ twisted leg.
Employee was assigned to stripping starter sheets. A starter sheet was sticking to the blank. Lifting up on the sheet to get a more leverage to pull down again the blank also lifted causing it to come off the hooks. The blank weighing about 50 lbs fell on the top part of employee's foot. The impact caused soreness to the upper portion of EE's left foot. Restricted for 1-day.
EE was throwing a switch at the bottom of the rail yard at Agrock when ee felt a sting in left shoulder.EE noticed stiffness to upper shoulder and when raising arm, ee applied cream but stated it's slightly worse. EE had MRI on 5/1/18 at which time was determined to be work related and put on restrictive duty.
Employee carried buckets or stone up some steps and spilled some stone on to the steps. On the way back down the employee slipped on said stones and fell on the steps injuring EE's low back on the steps.
Employee was driving up the employee parking lot ramp when the van skid and stopped on snow. The employee got out of the van to see what they could do and slipped on snow/ice. Employee sprained the right knee and ankle. The employee had not actually begun work yet when the accident took place.
EE was shoveling 1B Head when ee felt a pop in lower back. EE has returned to work on modified duty.
While lifting a worn screen panel the employee strained the lower right side of back.
EE slipped in the parking lot on 2/27/2018 spraining ankle and was released with restrictions but able to do job at full capacity. After a Dr. follow-up visit on 3/28/2018 and more x-rays it was discovered that EE had a slight fracture. We are filing this accident as MSHA reportable at this time after the x-ray diagnosis showed the slight fracture.
Employee was helping forklift driver by pushing the two forks closer together. When the employee was pushing the forks in, EE felt a pop in EE's knee/calve.
As the miner was stepping off of a 980 CAT Loader,ee stepped onto uneven, Snow covered ground causing ee to fall and break left Ankle.
EE reports that EE was doing a lot of sledge hammering yesterday in awkward positions. EE states that EE's lower back was sore but did not think anything of it until EE got home last night.
Removing 2X2 ceramic liners from the discharge box of #12 screen. I grabbed the heavy liner and set it outside the box on the catwalk. I realized it had serrated edge. Cut the palm on left hand.
Employee was pushing two batteries on a pallet and felt pain in the hip.
EMPLOYEE DESCENDING FROM LOADER WHEN EMPLOYEE SLIPPED AND FELL DOWN STEPS/LADDER AND HURT BACK.
EE strained lower back while picking up a 20lb bag of litter that had fallen off a pallet. EE placed on work restrictions & given transitional work assignment.
EEs were performing a full log washer paddle change, all were properly LOTO in the MCC. Logs were secured with a come along to prevent the weight of new paddles to spin the logs. The come along slipped, EE became pinned between paddles and the wall of the log washer, EE was freed quickly. The accident caused abdominal contusions, EE was prescribed pain medication and light duty.
After receiving the C-4 on 3/13/18 and completing an investigation it was revealed that the employee was throwing pipe down a shoot when one of the pieces of pipe got lodged on a hard spot and the force sent a shock up the employee's arm causing the strain
Employee experienced a rough load from Shovel causing neck to hurt

An employee was dismounting the bus when their knee locked up and popped causing them to fall to the ground.
Employee was operating a haul truck in adverse weather conditions (extreme freeze/thaw) that caused the haul roads to become rougher than normal. The employee experienced lower back pain.
Tailgate slammed shut on employee's left index finger fracturing finger and causing lacerations. Employee was pulling open tailgate with left hand trying to get rock out with right hand. The tailgate slammed shut on the left index finger. The employee was transported to Indiana Regional Medical center.
EE was helping the welder with a job and stepped off of C-2 conveyer frame and stepped on a rock and sprain right ankle.

EE drilling in sheet metal, bit hung up twisted hand/arm. No medical at time, later reported pain, had treatment. EE advised of strain shoulder/upper arm. EE returned, regular job duties. Follow up 3/9 added ""no operating heavy equip."" EE unable to perform the regular job duties. 7000-1 reported within 10 business days of new restrictions. Terminated on 3/14, unrelated to injury.
Employee stated that ee was throwing the track switch located on coal fork bottom so another ee could spur up a man trip. Employee stated that ee stepped over the switch and as ee threw the handle it flew out of hand striking left knee causing a bruise.
Employee was loading heavy pieces of steel into the back of a pickup when EE felt pain in the lower back.
On 2/22/18, Injured Employee (IE) tweaked back moving a wheelbarrow full of material during a housekeeping task. On 3/20/18, IE went to doctor because ee was not getting better. IE's doctor stated back is sore, ee needed time to heal, and placed ee on two months of restricted duty.
Miner was attempting to free up a frozen conveyor. Another miner above knocked frozen material loose. The material struck the miner on the right hand, breaking 3 fingers.
Employee was pouring concrete and using a concrete vibrator to spread material when wet concrete slashed and got under EE's safety glasses.
Employee hit pothole with JLG. Employee's knee struck pedal cover causing a laceration
EE performing hand scaling underground with a scaling bar and felt pain in right elbow. EE was sent for medical treatment on 3/14/18 and was placed on restricted duty at that time for elbow sprain.

Employee states that EE was struck with a continuous miner trailing cable that was torn loose from the rope hangers when the cable was caught by the roof bolter and was pulled, breaking the rope hangers.
Crane was holding the lock post cylinder 8""-10"" above the platform as the emp was kneeling beside it EE was hitting the lock post cylinder rod trying to extend it out when the snap ring holding the top cap in place came out and allowed the cylinder body to come loose, hit the platform and fall onto the emp knee.
Utility miner stepped over some 6"" pipe in the field and slipped on some ice twisting left knee. EE was placed on restricted duty.
employee was cleaning out build up material from hopper with a bar when EE hit EE's elbow on metal hopper causing lateral epicondylitis and contusion, Employee was not put on restrictions until doctors visit on 03/23/2018.
EE was feeding the system when EE was signaled by co-worker to come upstairs. EE stopped the skid-steer, set the parking brake and exited the equipment. The bucket was full of material. When EE stepped down from the skid-steer, right foot came down on a piece of material. EE came down on it in such a way that EE's weight shifted and EE rolled ankle.
The employee was removing a guard. EE was holding the guard with left hand and struck the guard with a hammer with right hand. The guard dropped down & pinched the small finger on left hand between a block and the handle on the guard. The employee broke small finger on left hand.
Employee was crouched down, below a piece of steel angle, working and stood straight up when finished. Hit their collarbone on steel angle breaking the collarbone.
Employee was using a impact tool to remove nuts and bolts from a loader bucket cutting edge when one of the bolt heads spun in their hand causing a laceration.

While descending ladder on dragline DL039, employee lost footing and took right arm to grab backwards to stabilize self and felt pain in right shoulder. On 2/26/18 employee refused medical attention. On 4/23/18 employee was taken to local Urgent Care for observation and put on restricted duty.

Employee was climbing up onto D9T dozer, foot slipped off of track grouser and caused scrape and deep bruising. The employee reported the incident on 2/19/2018. The employee sought medical attention on their own over the weekend. On Monday, employee was still in pain and was taken to the ER, where they were diagnosed with a bacterial infection.
Employee was working on coal mill tightening bolts on Thursday 2/15/2018. Employee states employee felt sore. Employee reported the soreness on Friday 16th and wished to seek medical attention. The employee was diagnosed with strains of both shoulders and placed on restricted duty.
Employee began feeling numbness and tingling in right arm. Initial cause was not determined. Medical treatment was sought and a prescription was given as treatment. After third party investigation, it was determined that the shoulder was most likely caused by repeatedly breaking chunks of frozen material to allow them to pass through the rotary dryer.
Employee was operating a forklift, moving refractory material in and out of a kiln. While backing out of the kiln, the wheels slipped at the transition of the brick ramp. Employee made several attempts to back the lift up and on one of them, jarred their back. Reported incident to Supervisor, taken to DR, diagnosed W back strain, placed on RD 2/16, Rtn to full duty 3/5.
Employee was checking for leaks under a loader using a creeper. EE rolled out from under it, stood up, and bent over to pick up the creeper when EE felt a pull in back.
Employee was moving a heavy plate (spacer) used to open a door. The employee felt pain when they lifted the plate with right hand but pain dissipated and completed shift. The following morning they reported pain in the hand/wrist.

Employee was operating a CAT 972M Rubber Tire Loader when EE felt pain in back. EE then reported this to immediate supervisor, who took EE to the Med Center to be evaluated. There was not a specific activity that triggered the back pain. EE was released back to work with restrictions. The employee has been a Loader Operator at the facility for 28 years.
Employee was lifting machine liners with forklift and sling, liner spun around and struck top of foot.
Loading a shot, began to have left shoulder pain, no known injury.
EE was changing the cement valves top of Grp 2 silo. The 1st bar valve moved hard. EE used full body weight on the 2nd and it moved easier causing them to be off balance. EE rolled R ankle. EE iced it and took ibuprofen. Worked for 2 weeks then decided it was not feeling better. EE went in for an x ray on 3/1. Displaced fracture R distal fibula.
Employee had completed cutting an H Beam on the band saw in the welding shop. EE picked up the cut off piece off the band saw that weighed 30 pounds and felt a pain in back.
While descending a shovel, employee was planting their right foot down on the next step and felt a ""snapping sensation"" behind the right knee.
Trying to break up frozen stemming felt a pop in left shoulder
Miner began to ascend front-end loader ladder, grabbed both side ladder rails and went to step on first rung, foot slipped off rung and caught fall with arms jarring right shoulder.
Employee was loading ore into a hopper using a Bobcat skid loader. The hopper was plugged so the employee got out to unplug it. When getting back into the Bobcat, EE's foot slipped off the step and EE fell into it hitting ribs on the right side.
IE and another EE were inspecting The VSI crusher. EE was on top pulling on the handle and as the door lifted IE was helping by pushing on the bottom of the lid. When the door was partially open IE thought EE had control and quit helping and the weight was to much for EE to handle. The door slipped out of EE's hand and fell onto IE's right hand.

Employee was trouble shooting a shield by engaging the solenoid controls on the shield, when a 2"" return hose came out of its fitting house due to there not being a staple lock installed and struck employee on the upper lip. The injured resulted in employee receiving 5 stitches.
Employee moving oil barrels twisted wrong and pulled muscle in back.
Employee was mounting Haul Truck and felt a pain in left shoulder.
Contracted employee started shift and went to climb into the crane. Contractor used the outrigger arms to access the cab. The contractors safety vest snagged on the hydraulic valve box and caused the contractor to fall. The contractor fell and landed with their shoulder on a job box. The contractor was taken to er by ambulance for evaluation. Fall caused a broken Clavicle.
On 2/14/2018 employee reported that on 2/10/2018 employee was operating a skidsteer while packaging mill concentrate and a gust of wind blew the concentrate in left eye. Employee was wearing a half-mask respirator and safety glasses. Employee then switched to a full-face mask and completed shift. Upon return to work, employee eye was red and irritated.
While using impact wrench and socket, socket fell striking employee on hand
A Mine Production crew was repositioning trim chain onto a sprocket, trying to rotate it by striking it with a hammer, so employee was kneeling in front. A fragment of metal separated from the sprocket and struck the employee in the upper leg. An attempt to retrieve it was done on 2/21;ee received two stitches and was placed on restriction to allow the injury to heal.
Employee was lifting timbers out of the bucket of scoop when they felt a pain in the lower back resulting in a sprain/strain injury and restricted work days.
The employee stated ee was in the process of opening a set of 6-foot airlock doors when the door ee was opening slammed shut causing the employee right to get smashed between the doors. The employee sought medical attention and was given restricted duty. This incident is being reported as restricted duty.
Mobile equipment mechanic was attempting to fill hydraulic oil in a bull dozer. The EE was standing on the bed of the service truck when EE turned to the left. The employee accidentally stepped off of the bed fell to the ground. The employee was diagnosed with a wrist strain/sprain and an elbow contusion. EE was given a brace for the wrist and was placed on restricted duty.
Repetitive motion while using a material handling vibrator, walking up and down stairs, to get to silo that employee was working on
Trying to step onto slab to put timbers on and tripped and fell causing EE to sprain wrist.
Employee bent down to lift a metal plate, felt a pop and pain in abdomen. Medical diagnosis showed to be a hernia
Employee was repositioning dust mask and lost grip. Mask popped against face causing dust to go under safety glasses and enter eye.
Employee was relieving a laborer for break and was lifting 55 lb bags of refractory onto the gunite machine for 30 minutes. After 20 minutes, employee felt a pain in the lower back.
Employee was pulling on a wrench to replace hydraulic hoses. Felt a pull in the right bicep.
Injured person was cleaning glass beaker in the lab with the incorrect size wire brush when the top of the beaker broke causing employee to cut right index finger.
Employee was cleaning the area of the Stage #4 duct. When EE hit the beam nearby, accumulated dust get into EE's eyes. The employee immediately washed eyes with water. EE was using the standard PPE.
Employee stated EE was putting a 40lb bag of rock dust into a pull behind duster when EE felt a strain in EE's lower back. Employee sought medical attention on 2/12/2018 and was diagnosed with a strain and given a full release. On 2/19/2018 employee followed up with family doctor and was placed on restricted duty.
While lifting stabilizer pads for crane, employee felt pain in employee's lower right side back

Haul Truck Driver was doing shift change after sitting idled for 11 hours. The employee dismounted haul truck and foot slipped on a rock and twisted employee's ankle.
Employee was removing heavy part (u joint) from under loader. Once removed, it slipped from employees hand and fell to the ground pinching their finger between the part and the floor.
Employee was rotating a hopper section when EE lost grip and the piece of metal dropped onto EE's right foot causing a contusion. Minor injury resulting in 5 days of light duty.
While cutting steel for a staircase the torch snuffed out and when EE went to restart the torch gas entered EE's glove and ignited. Another EE heard a pop and then saw EE restart the torch and then pulling EE's glove off like something was wrong went to see what happened and saw that EE burned EE's hand.
Miner struck a finger while using a 3 lb sledge hammer and a hole punch to punch holes in flashing while making repairs to the Fat Pak Crusher. This resulted in a crush injury to the left index finger and restricted duty.
EE was walking from the breakroom to the West warehouse. When EE walked across an area that had been leveled with 2"" rock EE lost balance. When EE fell and caught self with hands and suffered a contusion to left palm.
Employee was performing a clean up on the Coloring tank chutes to move granule from production tanks to shipping tanks. Operator pulled chain and felt strain in their neck.
A rock (football size) rolled down and off a catwalk, striking the ground person in the back as EE was bending over shoveling. EE went to the clinic and was put on restricted duty with a bruise to the lower left side of EE's back.
While adjusting a belt being installed, employee's right middle finger was injured as coworkers spun the sheave. Finger was caught between the belt and sheave.
Employee was walking when the employee slipped and fell. This incident did not become a medical incident until 8/17/2018
The employee was driving haul truck in underground mine and ran over rock on road. Reported tightness in upper back but thought EE was OK. EE later requested a medical evaluation. EE was see on 2/9, and was put on restricted duty. An investigation was completed and it was determined that hitting the rock may have aggravated an exiting condition.
The employee was walking into the electric shop and tripped on the step. Employee reached out to catch themselves and cut a hand on an adjacent sink.

Employee reported was lifting a water jug to place into a cooler when employee felt a 'pop' in right biceps area.
Welder was in the process of cutting a strip of metal into 4"" pieces using a metal shear. While shearing the piece of metal a tip-up occurred allowing a finger to be pinched between the piece and table that resulted in lacerations to finger. Brought to ER. Stitches received and x-ray showed a fracture.
The employee bent over to pick up a metal chute door in tower 1 of the screening plant. When the employee came to me ee looked to be in pain and was having trouble moving. Took employee to an urgent care where the doctor prescribed a muscle relaxer at the employees request. Put on light duty and went back to work on the same day.
Injured employee (IE) was backing a forklift down a concrete forklift ramp. When the forklift got near the bottom a piece of the concrete broke loose and caused a hole approximate 12 inches in diameter. The forklift tire went into the hole and the front end of the forklift quickly dropped approximately four inches. The sudden drop and stop jarred the IE and injured IE's back.
Employee felt pain in their hip when their haul truck bed was contacted by a loader bucket while being on loaded on 2/1/2018. Due to a reoccurring pain the employee was seen on 7/23/2020 and it was determined that they needed to work on restricted duty.
Employee suffered a neck strain when a sledge hammer head came off of a hammer, striking them on the hardhat. The hammer was being held by another worker as the injured miner was assisting loosening a bolt.
While operating the left side drill head of the RB-2 roof bolter, the starter steel became stuck in the mine roof. The operator lowered the drill box down 1/2 of the way and jerked the stuck steel out of the hole. The operator's small finger on left hand was pinched between the drill steel and the drill head of the machine. 3 stitches were required to close the wound.
Employee was helping another mechanic set a shuttle car fender back in place, when employee caught left ring finger under the fender causing a laceration that required 7 sutures to close the wound.

Employee was walking near the entry driveway of the quarry when EE slipped and fell on a rock.
Employee was sweeping and backed up and hit EE's elbow on the stairway. Talked with a nurse but did not seek medical attention until 3/13/2018. An MRI was schedule and medical release was given on 3/28/2018. Employee will be required to wear a splint while working EE's regular job.
An accident occurred in the E-30 Continuous Miner section. While the crew was mining in the #3 entry with #26 Joy ED CM they encountered a clay vein that liberated methane that was ignited by an undetermined source. The heat from the incident caused minor burns to the forehead and nose of the return side bolter.
Driver experienced pain in back after offloading dyed diesel. Driver stated after handling a fuel hose driver noticed pain in back.
Employee was exiting back of service truck and boot got stuck in step, and employee fell backwards twisting ankle, was diagnosed with ankle sprain returned to work full duty but was given physical therapy session 3/6/18, and scheduled a follow up 03/09/18 no work days missed
Laborer was palletizing rock and didn't move hand fast enough. Smashed left pointer finger between rock ee was holding and rock already on the pallet. EE was wearing gloves.
Employee was in the process of changing the hydraulic pump on a shuttle car in the shop. While pulling hydraulic hoses to be able to access the pump, EE felt a strain in EE's right shoulder down to EE's elbow. EE continues to work and started physical therapy on 3/7/2018.
Employee was tightening a ratchet strap, the strap slipped off and hit EE in the face causing a laceration to EE's chin which required several stitches.
An employee was pushing an empty cart into the passenger elevator on the preheater tower when EE felt a pop in EE's left elbow. EE initially felt it would be ok but pain persisted until went to a doctor who diagnosed EE with a detached tendon from EE's bicep. On 3/12/2018 EE had surgery to repair the problem.
An employee began to unlock the front gate of the facility, where EE received a 1/2"" laceration to palm while retrieving the locking mechanism/chain from a nearby rock. The injury did not involve any issues related to equipment, skills or miner proficiency, training, protective clothing or devices, or aspects of regulatory compliance.
A miner was hooking up a water hose to jumbo when the Dixon fitting struck and chipped front tooth.
EE was bolting in the No.2 Entry on the No.1 section. EE was installing the fourth row of bolts when a piece of draw rock fell from the top striking the EE on the left forearm which resulted in a laceration requiring stitches.
Employee was changing bit on blast hole drill. When EE reached to straighten the bit to connect it to the drill steel EE hit right little finger on a sharp edge of the breakout wrench. The sharp edge cut through the glove and lacerated EE's finger. The laceration required 4 stitches.
Employee was getting into position to cut bar grating and while getting into position the employee cut their leg on a protruding piece of steel.
While tramming a scoop, ee ran over a piece of rock and hit head on the roof. EE needed 6 staples.
EE lifted ee face shield after grinding to talk to ee supervisor and felt a particle enter into ee eye. EE was seen at the clinic and prescribed antibiotic drops.

Employee was installing drive roller. It shifted and pinched right finger causing injury
Tripping rods out of a 90 degree up hole, when the bell of the rods caught the wire and a piece of loose rock hit the back of EE's hard hat and then another hitting EE in the face causing a laceration under left side of nose on upper lip. The laceration is approximately 1 inch long.
While cutting off metal conveyor belt splices with a pneumatic cutting tool, the head portion of the tool got loose and spun around and the cutting blade disk cut the palm of employee's right hand.
Employee was working on a dust collector, unchoking a hopper when material started flowing out the poke hole as they attempted to close the lid, dust was ejected and got in their eyes. Eyes were flushed on site, employee taken to Dr and given prescription antibiotics as a precautionary measure. Employee returned to work with no restrictions- MEDICAL TREATMENT ONLY.
Employee was climbing over the pan line and hit head on a low hanging roof strap, which caused a laceration above left eye. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
Employee pinched right little finger between a scraper and a hopper wall while trying to scrape frozen sand out of hopper. The finger was first treated with first aid. Two days later the employee was seen by a doctor and there was a slight fracture to the finger tip. Employee was able to return to work with no restrictions on next scheduled shift.
EE was trying to get a cable when EE stepped off the concrete onto what EE thought was dirt and mud. EE stepped on a board with a nail in it that was under the dirt. The nail went through EE's boot and into EE's foot.
Employee was bent over, looking under a catwalk in the Page dragline and when ee stood up ee hit head. EE received a cut on the top of head that required 6 staples.
Employee said that EE was picking up a piece of concrete the size of a softball and right shoulder dislocated. EE was able to put it back into place by self.
Roughly noon on Tuesday, Feb 20, ee were on top of the new water tank checking the heat trace to make sure it was working. EE was at one inspection hole and as ee stood up to walk to the other inspection hole, feet slid out from underneath. EE landed on bottom and didn't think much of it the time. As the day progressed ee became more sore.
EE got foreign material in right eye while picking up safety glasses. EE received an abrasion on eye and given a prescription eye medication for treatment. EE released back to work on full duty following medical treatment.
REPORTABLE ONLY DUE TO SUTURES - Employee was roof bolting and as ee put the drill steel together and put pressure on the steel, the steel bent hitting ee in the face requiring sutures to lip and cheek.
EE was running a jackleg performing bolting when the pressure of the jackleg pushed on the basket of the JLG and tipped it towards the ground. When the basket impacted with the ground the jackleg struck EE in the right shin causing a laceration. First aid at the scene and EE was treated at clinic with staples and released. No restrictions or missed days.
Operator was assisting in changing screens at wash plant. Using a pry bar to pry the bar slipped pinching operator's finger against screen
Lowering DTH Hammer in to service truck using a winch line. While repositioning from vertical to horizontal for transport, the hammer shifted against truck bed. This action pinched the gloved hand of the victim as ee was attempting to guide the hammer in to position. the small finger of the left hand was caught between the hammer and truck bed surfaces
Employee was replacing the wheel studs on 605 fuel/lube truck, using a punch and hammer. Employee was holding the punch with gloved right hand when the hammer glanced off the punch striking EE's thumb.
Employee was shoveling under the wear belt when they felt dust enter the right eye.
Employee started to pick up a top roller frame, when roller frame was pulled by another employee catching ee thumb between frame and rail fracturing ee left thumb.
Two employees were lifting an estimated 100lb mini-power zone from the back of a flatbed truck. One of the employees hand slipped from its holding position, causing the edge of the mini-power zone to strike EE's right thigh. This resulted in a laceration to the employees right thigh which required 16 sutures.
Employee was jostled while seated (seatbelt on) in the cab of a haul truck while being loaded. On 3/13/2018 employee went to the Doctor where they were prescribed physical therapy making the event reportable.
The employee was using a hand crank to raise the crossover platform on the rail load out. EE then engaged the stop gear on the crank. As EE released the handle the stop gear did not engage, this caused the handle to free spin. The handle struck the employee on left index finger, the impact of the handle cut finger. EE was wearing gloves at the time of the incident.
Rigging froth cells to be hoisted, slipped on painted steel straining right shoulder.
Employee was in the process of installing new skirt boards in the R-4 tailpiece, after unhooking the chain that was used to place the new skirt boards on the belt, employee was walking on the belt back toward the tailpiece and slipped and fell backwards. They attempted to catch themselves, causing a small ""chip"" fracture in their wrist.
Employee was exiting the loader when EE slipped on bottom step and fell to the ground landing on shoulder / arm. EE went to LRMH for x-rays and returned to work the following shift. On March 20, 2018 EE informed supervisor that EE's physical therapist took EE off work until April 17, 2018.
The injured employee was attempting to drill out a plugged purge line with a large hand drill. The bit caught in the hole and cause the drill motor to spin around and strike the employee in the right cheek. On Monday 02/19/2018 the employee experienced increased swelling and sought medical attention where they treated employee for infection.
While driving EE's Damascus EE hit a cable bolt with EE's head knocking EE's hard hat off and cutting EE's head. EE required 9 stitches.
Employee was cutting worn belting for skirts with a utility knife. Employee removed their left glove on the last cut. Employee slipped with the knife and cut a finger.
Driver was hooking toe strap back to D ring on back of Dozer and sprained left wrist.
A fully trained miner was using a hammer and chisel to take apart a coupler when a piece of metal sheared off and struck in the left arm. The injury was not the result of any compliance issues nor were any protective guards involved. The accident was not reported until 04/03/2018.
Employee was moving rocks by hand, wearing gloves, and right middle finger got pinched between two rocks.
A maintenance employee was using a lock back utility knife to remove the lid from an oil bucket. While in the process of cutting the lid free, the employee's hands slipped and the blade of the utility knife contacted the back of ee left thumb. The blade cut through ee glove and caused a small laceration.
Employee was removing toad stool from parts crusher. Employee struck an easy-out to remove a wedged bolt from a socket. A shard of metal came off the punch at impact and entered the employees left hand through the glove.
Employee was roof bolting. While retrieving EE's drill steels from the roof, the last 2 steels stuck together. When the employee pulled on them to get them apart, one of the steels came back and struck the employee in the nose, causing a small laceration and fracturing EE's nose.
Employee was loading a belt scraper into a ride when the cradle slipped and injured employee's pinky finger.
EE was trying to remove a pinner steel hung in the roof. While pulling and pushing the steel to fit the pinner chuck, and hit the wrong control lever causing right ring finger to be pinched between the pinner steel and the pinner steel clamp.
Assisting a co-worker turn 80-c cross flow and got hand caught between cross flow and beam.
Employee was opening the elevator and ee hit right elbow on the door frame. EE has started Physical Therapy as of 3/05/2018. EE is now a NDL accident.
Employee opened a cap on a radiator while it was still hot. Steam and coolant sprayed out on the left arm and face. Employee was taken to the hospital. Employee has 1st degree burn on the face and 2nd degree burn on the left wrist.
Employee was making a cable splice and cut right leg
While descending the ladder of the 980H, the employee fell to the ground causing a mild concussion with a potential loss of consciousness.
Contractor employee was sitting in a training session, began to feel hot/light-headed, stood up to walk out the back door. EE walked about 15' and passed out falling to the ground and striking EE's chin on a near-by table.
EMPLOYEE WAS WALKING ON A LOADER CATWALK. EE DUCKED TO GO UNDER A SIDE MIRROR AND STRUCK HEAD ON THE MIRROR BRACKET. A BOLT ON THE BRACKET HIT JUST UNDER THE HARD HAT BRIM. EE'S HEAD SPLIT OPEN FROM THE BLOW, STARTING AT THE FOREHEAD AND ENDING AT THE MIDDLE OF THE TOP OF EE'S HEAD.
EE was loading belt on a belt sled in the mine yard and the nylon rope being used broke striking the EE in the face.
Employee was attempting to install a V belt on a shaker screen. During the installation, the side of the V Belt cut the employee's finger.
Employee stated was attempting to install a roof bolt that was partially bent, when employee activated the rotation to line the bolt up in the drill chuck, the corner of the bearing plate struck the employee in the right forearm causing a laceration. The employee received medical treatment (3 sutures) and returned to the mine for next scheduled shift.
Mechanic was removing a haul truck rim assembly utilizing a port-a-power tool that was being held by a sling. The port-a-power tool slid out of position, twisted and hit the mechanic just below the brow area. The worker was wearing safety glasses. The impact force of the port-a-power tool was deemed low energy.
The employee was removing a bucket tooth and retainer pin on a Cat 988 loader, employee missed the driver tool used to remove pins striking right thumb with a 3lb hammer. The thumb broke in two places near the mid-knuckle.
Employee was using a utility knife to cut rubber belting. The knife slipped and cut the employee's leg. Stitches were required to close the wound.
Employee was going to raise the lid up over the engine compartment and it fell on their right hand.
The employee was emptying a trash barrel into the dumpster when the barrel slipped further into the dumpster pinching EE's right ring finger between the barrel and the edge of the dumpster. The ring finger required three stitches to close the laceration.
EE was replacing the idler arms on a D10 dozer. EE was using a RAD torquing wrench on inside bolt. The reaction arm was pointing upward. EE placed right hand on reaction arm & left hand on the trigger of the wrench. When EE applied torque pressure to the wrench, the reaction arm rotated causing right hand to be caught/pinched between the idler & the reaction arm.
Employee began working on the truck around 9:30 am. Employee performed a mental risk assessment. Removed the drive shaft from under the truck and put on tandems to work on. 1 of 2 caps were almost installed (with hammer) when driving the second cap in the drive shaft yoke employee struck their thumb with the hammer.
Employee was loading belt framing on a diesel ride and hit right forearm on a piece of metal cutting right forearm. The laceration required two stitches.
Employee was attempting to hang a light on the #70 long wall shield using zip ties. While doing so EE was attempting to cut off the excess zip tie with a razor knife and slipped and cut the left hand. Employee received stitches to close laceration.
Employee was loosening a jam nut on a Rotex arm when the jam nut unexpectedly broke free causing the employees hand to slam forward into the rotex arm.
Loader operator was loading mud and rock when they raised the bucket the rear wheels came off the ground. When the mud slid out of the bucket it threw them against the windshield causing them to cut their head. (Raining and very muddy conditions.)
Unstocking a cable from disconnected box, cutting ground lead loose, EE's knife slipped & cut thumb. EE received stitches and returned to job that shift on full duty.
EE using utility knife to splice a cable and tool slipped lacerating EE's left hand. EE taken to physician and received sutures and released back to work with no restrictions.
Employee walked thru the underground shop and did not get low enough while walking under fire suppression system and bumped head causing laceration.
Employee was injured as a result of horseplay and an attempt to intentionally injure another employee.
Emp was clearing out a clay hopper with an air lance when EE struck a hand rail with EE's Lt. hand middle finger. Emp was wearing gloves. Emp pinched the tip of EE's finger between lance valve and rail causing a fractured tip. Lt. Hand middle finger fracture / splint.
Employee was removing a staple from the waterline, when a piece of rust fell off the top of the shield around EE's safety glasses, and into EE's left eye. It did not involve any equipment or compliance issues. It also did not involve job skills, training, PPE or protective devices. Employee did not seek medical attention until 2/14/2018, and was prescribed medicated eye drops.
Contractor employee was shoveling the belt at TG4 xc 7 when the employee lost their balance. The employee attempted to catch self and inadvertently contacted a top conveyor idler. This contact resulted in a laceration to left index finger. This laceration required stitches.
On February 9th at approximately 10 AM, Employee was plugging in the hydraulic press in the shop at Plymouth in order to press out the ""dog bone bushings"" from a CAT 740 Articulated Truck. When the employee turned the plug into the outlet they received a shock. Individual was taken to urgent to be checked out.
While exiting the change house at the end of the shift, the injured employee was bitten by a bat. Employee was evaluated at the local hospital and the health department was notified. The bat was tested for rabies- test was negative.
EE was cleaning dried (mostly) mud from around the pedals of the plant Bobcat loader prior to use...EE was using a trowel to dig the mud and chip away at the dried material...EE's hand slid down the handle and the tip of the right ring finger struck the back of the trowel...(4) stitches and released to return to work with no restrictions
Employee injured EE's heel while trying to get a crusher part.
ee was coming down a step ladder slipped striking ladder with EE's head causing a cut that required stitches treated and released no loss time
EE pulled the Tractor Trailer w/container up to seal placement rack. When on top of the rack there is a ""trap-door' in the catwalk. EE said EE placed the security hook to trap door to prevent it from falling. As EE was placing the seal lock on the manhole cover, the trap door fell down. the trap door hit EE in the back of EE's head pushing EE's forehead into the catwalk.
EE stated EE was moving a hanging scale and could not see so EE stepped up on the side of the hyster and laid hand on the boom and raised the boom and pinched fingers.
Employee was pulling a weld torch cart around a shovel stick in the shop. As the employee was walking and pulling the cart, EE struck EE's left ear on a 1"" piece of round stock located on the shop material rack.
A piece of draw rock became dislodged from the mine roof and fell approx. 3' before striking injured in the head and then in the left arm and hand. When the draw rock struck injured in the left hand palm side thumb area EE received a laceration. Inj was transported to Clinch Valley Medical center where EE received 10 sutures.
The electrician was removing de-energized power cable from #1 portal. While using a cordless band saw to cut the cables the electrician's hand slipped making contact with the blade. The result was a laceration to the left index finger.
Slippery road conditions, Employee was hauling an empty truck in route to be reloaded when the truck started sliding and upon employee counter steering, the truck went into a ditch, getting in a sump.
In the process of changing the clean coal tubes the employee flopped the gate from tube 2 to tube 1. EE hand and glove on right hand got caught on the flop gate handle and the tube. This resulted in a cut to hand. EE was taken to the hospital where ee received 8 stitches and returned to work. EE has missed no work.
Field Helper was getting EE's equipment ready for the shift. EE was attempting to cut a pulling strap with EE's knife. EE's knife went through the strap too quickly and made contact with EE's hand. The injury required medical attention and stitches to close the wound. EE was cleared to return to work with no restrictions.
The EE was grinding on a jaw box rebuild with an electric 4 hand grinder with dual eye protection. EE felt something get into right eye, used eye wash to flush it and finished the shift. EE came in the next shift, reported discomfort still. EE was taken to the clinic and diagnosed with an eye abrasion/given prescription eye drops/released with no work restrictions.
EMPLOYEE WAS WELDING AND OTHER CONTRACTORS WERE ABOVE EMPLOYEE, GRINDING. EMPLOYEE WAS WEARING PPE BUT A PIECE OF RUST BECAME EMBEDDED IN THE RIGHT EYE.
A contract welder was using an angle grinder to cut some material on an impact crusher. Upon completing EE's cut EE proceeded to set the grinder down and it twisted such that the grinding wheel cut through EE's glove on EE's left index finger causing a laceration of about 1"" in length.
Employee slipped and fell into a pile of small sharp rocks resulting in a laceration the required 4 sutures.
Employee was removing a welding strap approx. (2""x8"" & 3/8"" thick) piece of metal from cable crossing skid. EE used a 4 pound hammer to try and remove the strap that was tack welded and it broke loose and contacted right index finger. This caused a cut which required stitches.
EE was hammering on a piece of metal, trying to straighten it out. EE missed swinging at the metal and strained EE's right wrist. An incident report was filed on but no medical attention was administered. EE aggravated the wrist again on 3/12/18, and sought medical attention on 3/13/18.
Using a wrench to loosen up hydraulic line on HP400. The dust cover slipped which pushed their hand and the wrench into the main frame and caused a small stone to be smashed into the side of a finger.
Employee was making a handle for a tool box, as ee was bending a piece of metal, hand slipped and the metal stuck in left hand, went to Harlan ARH ER and got (7) stitches.
Fumes from welding pipe caused chemical burns on eyes of employee. Employee should be off for 7 days according to Doctor.
Employee was carbon arcing inside the materials dryer. A piece of rolled off the welding jacket to EE's groin area causing burns to EE's genitals.
Employee was trying to adjust rail ties in the 8 South mains track entry when rail shifted pinching left ring and middle fingers resulting in lacerations. The lacerations were treated with dermabond. Cause of injury was awareness Compliance with rules/regs,PPE,mining equip/systems not a factor. Miner proficiency may be a factor.
Miner was installing a bearing set beam in the shaft at the 1-5 level, miner was using a spud wrench to align the beam when the wrench slipped out of the beam and caught a fingertip between the beam and wrench.
Maintenance employee was conducting routine work of rotating cutting edge on Cat 966K Loader when worn carriage bolt spun and caused a laceration on the left middle finger of the employee. Standard leather gloves were being worn.
While taking canopy post off, the cylinder collapsed and caught EE's fingers between the post collars. Received stitches to left index finger.
Employee was grinding off metal that had been cut on flop gate. A piece of metal must have got under EE's safety shield and safety glasses, causing an abrasion to eye. Employee did not notice discomfort until EE got home.
Employee was loading crib blocks when EE's little finger on EE's right hand was mashed between two blocks, as they rolled on EE's finger. EE was taken to the hospital where EE received 4 stitches. EE returned to work with no restrictions.
While changing battery on green ram car at #1 Battery Shack the charging lead had a splice in it and when taking the cathead (plug) out of the battery receptacle the employee said ee was shocked on the right forearm. The employee was taken to the hospital and checked out and received a return to work full release paper.
Employee was walking to the carousel of the bolter when EE slipped, EE contacted the rib causing a laceration to right ear. EE was seen by site medic and referred to the ER, sutures were required, A full duty release was issued making this a medical reportable incident.
Dusty conditions, PPE ( safety glasses were provided and in use)
The employees drill steels cross threaded. While trying to correct the problem the drill steels popped and struck the employees left hand. This caused a laceration to the left thumb. (2) sutures were used to close the wound.
Reportable due to sutures. Employee was trimming belt when the knife EE was using slipped cutting employee above right knee.
Employee was throwing an extension cord up to another employee when lanyard got wrapped in the cord and the lanyard swung and struck the employee above right eye.
The employee was walking to assist with a cable pull when a small rock fell through the wire and bounced off the Ventilation duct and struck the employee on the upper lip. The employee received a small laceration requiring sutures.
EE removing a broken door glass from the door frame with a scraper tool when the tool slipped lacerating right hand. EE received 5 sutures on the back of the right hand. EE was placed back to work full duty following medical treatment.
EE was working on the interior of the roaster grinding teeth off of the roaster arm. Sweat had ran into EE's eye causing irritation. EE rinsed EE's eye, however it was still irritated. EE was taken to a medical facility where EE was treated with antibiotics for an abrasion. EE was released back to work.
EE diagnosed with Carpal Tunnel Syndrome on 5/4/2018. EE remains full duty pending treatment/surgery. If EE has surgery, EE will be released to restricted duty following surgery. Treatment and/or surgery is pending at this time.
The mine received Self-Insured Employer Decision on 2/27/18 from a medical report dated 2/19/18 awarding the injured a 0.37% permanent partial disability from occupational noise exposure. Employee has missed no work due to this occupational illness. 2 years of 30 years experience have been at this mine.
