Accident type, without injuries

Our permitted landfill built on an old waste rock dump had an unplanned fire occur. The source of the fire is unknown. No damage was caused by the fire and no personnel or equipment were exposed.

A roof fall above anchorage occurred on the active 001 MMU. The fall occurred in the intersection of the #2 heading four crosscuts on to the #3B Panel. The fall was dangered off with timbers and cribs. A 35C scoop was covered up by the fall.

An unplanned roof fall was discovered in the Main North Intake #2 Entry at 139 block. The approximate dimensions of the roof fall area is 30' long x 16' wide x 7' above the coal seam. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

Found an unintentional roof fall while making the weekly exam in the 1st right off of Orchard Branch Mains.

#3 Hoist skip and gate proximity switch had malfunctioned. Evac all ee's out #2, all out safely. Repairs completed at 3:15pm.

The small two-man hoist located at the Camp Creek portal was taken out of service due to a bad place found in the hoisting cable during the required yearly x-ray of the hoisting rope that was conducted on January 24, 2021.

One miner was trapped on Miracle Run Elevator while riding down to the mine. A normal positioning switch faulted causing the elevator to stop. The miner got off the elevator at 1:54 PM, and Schindler Elevator Services inspected the elevator prior to going back in service.

A roof fall measuring approximately 55 inches high, 20 feet wide and 40 feet long, fell in crosscut between no. 4 and no. 5 entry near spad no. 2001. (The fall has been dangered off and will not be cleaned up)

Cagers stopped at the 4900 #3 Station, exited the cage to call hoistman. Cagers told #3 Hoistman that they would be working on 4900 station and they would release the cage after they unloaded their gear off the cage. While one cager was unloading gear the cage moved up with doors open damaging the doors when they contacted the overhead cap. No injuries reported.

On 1-21-2021 a non injury roof fall occurred in the area of spad 183.This area was cribbed off in all approaches. This fall was not immediately reportable.

There was a spot in the Material Hoist cable that had broken strand. The hoist was downed and the rope was shortened to remove the bad spot. The hoist was tested and put back into service. There was no accident/injury/illness in this.

High CO reading indicated a Coal smolder in Coal mill baghouse. CO2 and Fire Suppression required more than 30 minutes to extinguish smolder. CO & heat source identified as out at approx. noon. See Root Cause Analysis for full details.

We had a roof fall in the B 2 3 left section crosscut 3-2 Left Butt, Approx. four foot coal seam. The dimensions of fall is ""80' long, 18' wide, 10' high"". The fall appears to have occurred as a result of adverse rock associated with a geologic transition.

There was a strand found to be broken on the hoist rope. The hoist was taken out of service, the rope shortened, tested, and put back into service. MSHA was notified on property when the strand was found.

A brake shoe on the ore hoist in the #3-16' shaft was dragging on the drum during hoist operations. A heat sensor on that brake shoe detected a temperature rise above set limits and stopped the hoist. The incident was called in to MSHA because the response time of a mechanic to investigate the event and adjust the brake was outside of the Part 50.10 reporting time limits.

A roof fall was discovered in A entry At 33+18 7 seam A sub main. The roof fall measured approximately 17 feet wide by 70 feet long.

The Dry Ridge elevator was taken out of service by mine management for and extended period of time, due to the elevators fault alarms showing the elevator was not operating normally. The rope Gripper switch was changed along with the bottom door switch and the elevator became operational. There was no one on the elevator when it was taken out of service.

There was an unplanned roof fall in the 6th East #1 entry at spade 14+80 at cross cut 20.

The drive motor for the cage hoist failed resulting in the cage hoist to be down for operation until backup motor could be installed.

The control system during start of shift hoist checks. A pneumatic relay on the 16' ore hoist brake control system was responding slowly due to the cold weather. This slow response was detected by a brake system sensor causing the hoist to trip. Overnight temperatures at the site reached the low 30's that night.

The rope gripper mechanism malfunctioned which caused the elevator to stop, trapping an employee on the elevator.

An unintentional roof fall occurred in the 3 North Mains at 187 Wall 2 to 3 Crosscut. The fall measured approximately 16' high x 16' wide x 35' length. The immediate roof consisted of laminated shale material plus adverse geological conditions contributed to the failure. No injuries occurred as a result of this incident.

An ignition occurred at shield #58 on the E-7 Longwall panel face when the shearer was attempting to cut through a sandstone intrusion that had dropped down in into the coal seam from the top. When investigated by MSHA and company personnel, only one worn bit was found that might have caused the ignition to occur.

The Miracle Run Elevator was down for a period of time when called to the elevator bottom. A piece of ice fell from the coping on the warehouse side, approximately 20' down the shaft. Damaged stainless steel top and conduit going to door locks and slow down switches. Repairs were finished approx. 3:00 AM on 1/15/21. Also, went down again at 8:22 AM on 1/15/21 for door switch.

Due to a large wind storm, the mine lost power and this caused both hoists to become unusable.

Bottom south wheel assembly on the south skip, east wheel became misaligned causing contact with guide plates, timbers, and beams causing the bracket to bend. This prevented full movement of the south skip to the loading position. It was determined that tension on the wheel assembly may have been too high. Inspection and adjustment procedures are being reviewed with all shifts.

A windstorm caused loss of electrical power to the mine. The mine did not receive power until 0901 hrs. January 14th. 78 miners were directed to stand down at the shaft level stations. All miners safe and accounted for.

The hoist went down due to a power bump, it caused a YA1 hoist drive to trip. The YA1 was reset and the hoist started to work again.

A roof fall was discovered during the morning preshift in the face of #3 entry in 14 Butt (two crosscuts inby spad #6003). The fall measured approximately 18 ft by 75 ft by 6 ft. The fall appears to have occurred as a result of a cutter that formed in a geologic transition zone.

When checking the upper limits switch on the service hoist, the hoist operator brought the service cage up into the headframe without de-clutching causing the counter weight on the other side to drop out of the guides. No one was in the cage, this was during pre-shift checks.

During a general hoist inspection On January 12, 2021, 2 broken wires in a single strand were identified on the head rope for the #1 service hoist. Two broken wires in a strand meets retirement criteria and the hoist was taken out of service to change the rope. The rope is a 6x25 flattened strand fiber core hoist rope.

A roof Fall occurred in MMU 001-0 at survey station 1470 in #3 entry. Approx.18 feet of coal was mined at the face of #3 and the MBC pump went out. The unsupported roof was hanging for approx.1hr.20 min. Time and poor geologic conditions caused the unsupported roof to fall and rip out previously installed bolts. Outby the fall was bolted with 6 FT. bolts and 10 FT. Cables.

Upon post blast inspection of the 82 655 +30 East heading, a fall of ground was identified in the area a slab round had been drilled and shot the shift before. The area was barricaded and reported to management. A call to MSHA followed.

Production Hoist #3 kicked out on tail rope switch fault. Working to resolve issues. Operational at 6:16 p.m. called MSHA and let them know.

The brake on the service hoist became out of adjustment when the hoisting engineer was doing the hoisting checks. The brake was adjusted after trouble shooting and placed back in service.

Unknown indigent arsonist started fire on outside of office building. Building totaled. No injuries to employees or anyone else. No one was present at time of event.

One of the trailing cables became dislodged due to normal wear and was pulled out from the mid-shaft junction box. This cable was replaced in its entirety and re-entered in the mid-shaft junction box to the elevator. No injuries had occurred.

WHILE CUTTING PARTS ON WELD SHOP PLASMA TABLE, EMPLOYEE NOTICED SMOKE COMING FROM DUST COLLECTOR EXHAUST DUCT. FIRE WAS IN THE COLLECTOR AND WAS EXTINGUISHED. NO INJURIES OR DAMAGE OUTSIDE OF COLLECTOR HOUSING.

An unintentional roof fall occurred in 7 North Mains 35 1/2 to 36 Wall #1 Entry. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer in the mine roof.

The 16 ft hoist is currently down due to brake issues. Troubleshooting is currently taking place, so repairs can be made. There are two other means of egress from the mine and no one was injured as a result of this occurrence.

A roof fall occurred on the HG9 travel road entry number 2 at 31 cross cut. The fall was approx. 12 feet high, 20 feet wide and 18 foot long.

At 9:41 am the silver shaft hoist went down due to electrical issues. The crew was put on stand down while the electricians diagnosed the problem. The electrician determined that it was a programming issue with the limit sensors and corrected the problem. The hoist was back up and fully operational by 11:50 am

#3 16'-shaft hoist tripped while conducting safety checks. Electrician checked control system and reset breakers more than 30 minutes after control system hoist tripped necessitating reporting to MSHA per 50.10 as defined in 50.2(h)(11).

A ground fall occurred post blast at Vista Underground-Turquoise Ridge at the 1N3960 level. No ventilation or access was restricted. No injuries sustained.

Struck by powered moving object

#2 shuttle car was traveling back toward #5 entry to turn down towards feeder, and the #3 shuttle car was already traveling this entry when the #2 car came thru the fly pad they contacted the operators compartment of the #3 car injuring the operator. The EE died on 2/21/21, autopsy results state EE died as the result of a thromboembolism.

Haul truck driver was dumping material at dump site. Truck went over the dump site and overturned at the lower level. Crews responded and notified Emergency Response. First aid was provided until EMTs arrived.

A miner was fatally injured while using a tool to remove a down hole hammer. The drill motor turned unexpectedly, pinning the driller's leg between the tool and the drill mast.

Employee's finger was pinched between the door of a loci and the rib. After EE called for the train to move EE reached up to close the loci door and finger was pinched against rib. EE was seen at site medic and eventually transported to a clinic where EE had surgery to treat injuries to two middle fingers.

While trying to put on a belt clamp, the tensioned belt broke loose catching employee's left thumb resulting in amputation.

Employee stated that they coughed and felt a sharp pain in their groin area, but didn't leave work and stated they felt improvement the next day. Employee ended up filing workers comp paperwork that was accepted on March 3rd 2021.

The employee was troubleshooting the main hopper belt. The employee removed the equipment guarding to check the motor and gearbox. The employee touched the v-belts for tightness and the v-belts began to turn, pinching the finger tips between the v-belt and motor sheave.

Fall from ladders

The employee was on a step ladder when the employee lost footing causing employee to fall 25"". This caused the employee to hit the concrete floor which caused a whiplash effect. The employee struck the back of their head.

A fully trained miner was removing a pin from a draw bar when the miner felt pain in shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

On 1/30/21 the IE was trying to free up material in the Mill 13 bin using a long rod to poke the material. EE felt a strain in EE's lower back and reported the incident to management. The IE continued to work until EE's doctor took EE off work on 5/18/2021

A fully trained miner was handing a come-a-long through a mandoor when the miner felt pain in lower back. The injury was not the result of any compliance issues nor were any protective guards involved.

While pulling the right middle shuttle car's trailing cable and anchor, employee felt pain between the shoulder blades.

Over-exertion in lifting objects

Employee was loading supplies on a supply trailer when EE felt a pop in the left shoulder. EE continued to work until 4/14/2021, but it was not until 4/14/2021 that workman's compensation determined that this injury was related to this incident and not a previous one thus making it reportable at this time.

Employee was attempting to move tubing over to align with existing tubing, when the handle employee was pulling on broke free from tubing and employee tried to catch self, fracturing left wrist. EMPLOYEE HAS BEEN WORKING SINCE DATE OF INJURY UNTIL 3/3/21 WHEN EMPLOYEE WAS TAKEN OFF WORK.

Employee stepped on the step of the powder truck to change the wiper blade, fell to the ground hurting right leg.

Employee was in the 9 West section attempting to free a bundle of rib pins from on top of the miner, when EE smashed EE's left thumb after striking it on the miner. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

EE stepped on a switch throw cover and slipped. EE felt a pop in the left calf causing swelling and pain.

Employee was walking to the truck line from the shop building, slipped on the snow covered ground EE was walking on and fell. EE injured their lower back and has pain in the right shoulder.

While changing screen media on a 6x20 finish screen, the 3 foot ply bar slipped, and caused the employee to lose balance and land awkwardly on right knee.

The injured was attempting to throw a track switch at Brk 19 on 10 East Section. EE placed left foot on the switch handle to push it down and when EE pushed the handle down, EE felt a sharp pain in the left calf. The employee started missing work on April 29, 2021.

Caught in, under or between a moving and a stationary object

Employee was operating Shearer in the G2 longwall. EE's left hand was resting on the handrail. A rock came up the pan line from the tailgate contacting EE's left hand. When EE attempted to move hand out of the way it became pinched between the rock and the top of the shield causing lacerations to EE's left fingers. EE received 6 sutures.

Ee reported being jostled in cab of haul truck when rock hit the headache rack. Seen at clinic and diagnosed with thoracic and lumbar strain. On 2/12 investigation complete and claim accepted as occupational making this reportable. Ee released to restricted duty.

Stuck material from first loader bucket into main trailer landed harshly jarring the truck. Afterward, driver reported pain in EE's lower back.

Employee was climbing down from ground ladder on crusher when foot slipped and rolled ankle.

Employee was loading supplies onto scoop when they stepped into a hole and tweaked left hip. Employee was wearing all required PPE and was fully trained. The employee did not seek medical treatment and worked until 2-19-2021, at which point the doctor put them off work.

Struck by falling object

I.E. was cutting a crosscut with the continuous miner. A piece of rock measuring 2'x1'x4in thick fell from in between the bolts. I.E. deflected the rock with their arm and it landed on their right foot behind the steel toe causing a fracture in the foot.

Removing & Replacing Bucket teeth, a shard of metal came off and went into right thumb damaging an artery and possible nerve.

Over-exertion in pulling or pushing objects

On 01/26/21 IE injured left shoulder while pulling a piece of curtain off top of scoop to put in dumpster. Unknowing to IE there were RB plates on the curtain that added resistance jolting the IE left shoulder causing a ""popping"" as it was pulled off. IE had surgery on 05/13/21 to repair a tear in the Rotator Cuff.

While walking past a screening system the guard popped off and hit teammate in the back. Teammate was placed on light duty effective 2/9/2023. Update: As of 3/06/23 this is now a lost time incident. Teammate will be off work until further notice.

Over-exertion in lifting objects

Injured employee was lifting a small pump out of a sump inby the 4 West #1 Belt Drive when they felt pain in right shoulder. Employee failed to use good lifting practices, over exerting shoulder. Did not violate any regulations and was wearing required PPE. Employee began missing work on 1-30-21 due to this injury.

The employee had just arrived at the Dry Ridge portal and was walking from their vehicle to the bath house when they slipped on ice, falling and landing on left elbow. X-rays would later reveal a fracture of the ulna bone.

Miner was replacing a track pin in a crusher. EE smashed a finger when EE swung the 3rd time with the hammer. Taken to emergency room in Ashland, KS.

A frozen chunk fell down approximately 20ft. into the cab of the excavator that was working at the toe of the face. The front post of the cab was pushed into the operators leg pinning them in the machine. The operator sustained a fractured sternum as a result of the cab shifting.

The employee stated the employee was in the process of installing the last gate shield when a piece of cap coal fell between the shields from the top and struck the employee in the right shoulder at the base of the neck. The employee was taken to a hospital, where the employee was diagnosed with a closed C-7 fractured vertebra.

While walking in the 9th North belt entry, the employee tripped over a belt tailpiece anchor pin and and landed on left knee resulting in knee damage that required surgery.

The employee stated: was in the process of installing the last gate shield when a piece of cap coal fell between the shields from the top and struck the employee in the top of the head. The employee was taken to a hospital, where employee was taken off work.

Employee had installed inside bolt of second row and was in the process of drilling the hole for EE's middle bolt. EE's steel had stopped up and EE removed the bottom piece to clear it out. EE re-installed EE's steel and was pushing it up to finish drilling when a rock struck EE on top of this left forearm resulting in a laceration and sutures.

Employee stepped through a door to get out of dusty beltline. Left foot rolled on a piece of loose coal as EE stepped through the door, twisting knee. Injury was not the result of a training or compliance issue. Employee continued to work, and began missing days as of 3/19/2021.

Today, January 26, 2021, the employee indicates that when employee finished work schedule on Sat., January 23, 2021, employee cleaned the cab & the ladder of the truck with water. As employee was getting out of the truck, employee slipped & grabbed the handrail. In those moments employee felt pain in left arm & back. The employee did not report to work on Mon., January 26, 2021.

EE was removing bolts from a shaker motor when EE felt a burning pain in abdomen. EE worked till 02/23/2021 when EE went off work for hernia surgery.

Employee hit left knee on the head of the bolter causing injury

Over-exertion in lifting objects

EE had removed a knot out of the miner cable and waterline. Afterwards, when EE reached down to pick up the slack rope to hook it to the CM EE felt a sharp pain in their back. EE continued to work until the date of August 9th when EE was put off of work from a doctor.

Employee was in the 9 West Section taking a cable out of a roller hanger when the hanger broke free, causing the cable to strike employee on the right shoulder/back area. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was at inch tram controls on roof bolter when a piece of roof rock struck employee in the right arm causing a laceration, resulting in multiple sutures.

Over-exertion in lifting objects

Employee overexerted while moving a stick of rail with a stone bar on the 8 West Section. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Flash burns (electric)

Miner (Service Technician) with amp meter inside live 480V Electrical Panel performing maintenance on air compressor without proper PPE. Unknown short across bottom of motor starter causing arc flash while miner was in front of electrical cabinet. Miner received burns to hands and flash burns to eyes from arc flash.

Fall onto or against objects

A fully trained miner was walking past the miner when EE slipped and fell hitting EE's arm on a rib strap. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was shimming up cat track on feeder, when a grease fitting came apart striking employee's left elbow causing laceration requiring requiring sutures.

Plowed snow turned into ice. When the operator walked out of the plant the operator slipped on the ice and sprained EE's ankle.

In the process of removing a hydraulic ram, I.E. was struck in the nose when the ram became dislodged from where it was placed causing a facial fracture.

Employee was walking from EE's vehicle to the time clock to punch in to begin EE's shift when EE slipped and hurt EE's shoulder.

EE was changing out primary flume screens at washer, EE felt a slight discomfort in right knee. EE was initially seen by EMT and reported to SC clinic on 1/21. Ortho evaluation on 1/27 possible torn meniscus. EE will have MRI and be out of work until further notice.

MINER WAS WORKING ON A PIPE, PARTIAL PART OF A DIRT WALL FELL AND HIT THE MINER'S LEG.

Employee was beating out a plugged drill steel and accidently hit EE's left index finger.

While traveling to section on mantrip employee felt a jarring in neck, resulting in a lost time accident

A fully trained miner was hanging the miner cable when EE fell backwards and injured EE's back. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was on Whittaker bottom and bent over to pick up a jack during a pre-op exam when employee felt pain on the right side of lower back. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Struck against stationary object

The injured EE was hanging a vent tube in the face of #1. EE was standing on a bucket and lost EE's balance. On EE's way down EE reached for something to catch self. EE grabbed a roof strap inside the walk-way bolt hole and cut EE's left middle and ring finger. Received 6 total stitches, and began missing work on 1/20/21.

Over-exertion in lifting objects

Employee was picking up a case of 6 hams and 2 turkeys. While loading hams and turkeys into the tailgate of the truck, EE strained back.

EE did not follow safety procedures of lockout and tagout, and proceeded to adjust door on hopper while conveyor belts were in motion.

Struck by flying object

There was a rock wedged between the dual wheels on the lube truck. After a few attempts at trying to remove the rock, the employee leaned over the tire and the inside tire blew out. The employee suffered a broken nose and facial bruises.

Employee was loading a 773 haul truck with the 988K Cat. loader. While loading ore into the truck, a rock slid over the back of the bucket down the arms and into the cab crushing the dash into the operator's ankle.

EE tripped in the clinker cooler and fell on left knee and sustained a 1.1 cm fracture to the knee cap.

EE was lowering gate, it got stuck on the rib, and glove caught on a wire. While pulling out the glove, the gate fell, striking the EE on right collarbone. EE was diagnosed with a fractured right collarbone and will be having surgery to repair on 1/22.

Employee was walking back to service truck at Marker #5 of the Workman South haul road. Employee fell because of snow/ice and landed on left shoulder. Employee continued to work till date of surgery on March 9, 2021. Employee is now placed off work.

Sometime in January (employee is unsure of date), employee was lifting a steel beam. EE picked it up and put it on EE's shoulder. EE felt pain in EE's left shoulder. The pain persisted for several weeks, but EE did not report the injury until April. EE had EE's shoulder checked, then had an MRI. It appears EE will need surgery.

Miner was stockpiling material away from the mortar sand stacker belt when the 988K loader EE was using came in contact with the belt. The belt was pulled onto the loader and the head pulley came thru the front windshield of the loader as it fell. Miner injured EE's left foot when the dash of the loader compressed onto EE's foot. Miner had surgery on foot to correct the injury.

Roof bolter operator was in the #2 entry putting up the inside roof bolt when a piece of roof rock fell striking the employee in the back between the shoulder blades. Employee is currently off of work.

While cleaning up coal bins at bagging department employee was shoveling coal and felt soreness to lower back area. EE was taken to local Urgent Care for observation and diagnosed with muscle strain to lower back and placed on restricted duty.

The EE was working on what will be 3 West Belt Line hanging belt structure. The EE picked up one side of a bottom cradle to hang it in a chain, when EE felt a sharp pain in EE's right shoulder. EE finished shift and began missing 1/15/2021.

Victim had stepped onto the top of a truck tire to gain access to the engine compartment. EE's foot slipped as EE was getting down and EE injured wrist when catching themself. Incurred fractured/dislocated left wrist.

IE was assisting in removing flooring in a basket. IE was prying on a piece of the floor that had been cut. When it broke free the IE hit hand on the metal basket. It was still hurting the next day so they went to get checked out.

IE was roof bolting. When IE was reaching to get drill steel from the mine roof a rock fell striking IE's hand. IE was brought to the surface and taken to have the hand checked.

The employee was reclaiming belt structure and pulled something in EE's left leg lifting a 42"" top roller. EE continued working but was still experiencing pain and discomfort on the back of EE's leg. An MRI later revealed a partial tear in EE's hamstring. EE is expected to make a full recovery without requiring any surgery. EE began missing work on 01/26/2021.

While moving longwall chain in a tub, the pry bar the injured miner was using slipped and EE's finger was caught between a chain flight and the chain tub. Injury was not due to a training or compliance issue.

Miner slipped from the 2nd rung of 3 foot ladder when EE lost grip while tightening a bolt on a pump. EE fell to the concrete floor and suffered bruising to lower back and hip. Ladder was dry but 3 points of contact had not been maintained.

Employees were working on top of surge tunnel setting form work in place to add decking for a concrete pour. The main beam on the form work broke causing the form work to collapse. EE was cleaning when bucket with lime bumped a step causing lime to go into the air and enter EE's eyes.

Fork Lift operator lifted an inner drum using a cable for securement. Employee tried to shift the drum on the forks from the side when it fell off and landed on right foot.

EE sought medical attention on March 1, 2021.  An MRI was performed on March 10, 2021 and EE was diagnosed with a rotator cuff tear.  EE will work full duty until EE's surgery date, March 19, 2021, which will then become a lost time incident.

Employee was pulling an unloading hose (4"" diameter) from Bay #2 to Bay #6 the hose caught on an I beam. IE turned to see what the hose was struck on, pulled hard, turned to continue toward Bay #6 and tripped over another hose. IE fell on left knee. EE was moving the cylinder to go between the bones of the truck when the cylinder tilted out toward the ladder, hitting the ladder and causing the EE to fall to the ground. EE seen at medical, diagnosed with a fracture to T3 and referred to ortho. EE released to light duty by ortho.

Miner, plant operator, was attempting to free VSI crusher of debris from the grizzly. EE did not follow shut down procedure to perform said action; left conveyors running, which resulted in EE's jacket becoming entangled in head wheel shaft of the conveyor, causing EE to fall, resulting in a broken arm, which required surgery.

While climbing down the ladder of John Deere pay loader the employee slipped on the second to last step on the bottom and slipped and fell. The employee hit EE's back on a wheel chock that was on the ground.

The employee decided to take a break outside the scalehouse area, and leaned up against a metal chain connected to a trailer building and scalehouse structure. Employee was removing a dust collector bag, it became stuck, as EE tugged harder, had a pop in shoulder. Self-care measures did not help, seen by dr on 1/11/2021, injection and restricted duty.

Employee was on knees on FM1 beating the hatch loose on the secondary end so we could drop and pull this hatch to spin balls out. As employee was swinging down, index finger on right hand was pinched between the sledge hammer handle and a liner bolt that was just to the north of the hatch.

Mechanic was working in Raw Mill tapping out bolt holes. While doing this EE felt a pain in the upper groin area. EE did not go to the doctor at first--thought it might go away. On 2/1/21 EE saw a doctor that said EE has a strain in EE's upper groin area. Dr. prescribed antibiotics and told EE to return to work with weight restrictions.

EE was stowing the outrigger pads for the RT crane. As EE lifted an outrigger pad (Pad weight was 86lbs.) EE heard a loud pop in their left shoulder. EE was taken for medical evaluation where EE was scheduled for an MRI and released to work with restrictions.

On 01/28/21 the employee was working to free a stuck cyclone in the vertical raw mill by using a shoulder to push against the equipment. In doing so the employee strained neck and shoulder and was issued a work restriction.

Stitches were required to close the lacerations.

An employee was helping with repairing the horn on the dozer. When they went to dismount the machine, the employee grabbed the handle to the hood. The hood shifted, causing the employee to lose their balance and fall between the dozer tracks and blade. The employee injured their leg.

Employee grabbed the hand hold with right hand and heard a pop in shoulder. Employee was put on restricted duty for shoulder strain to prevent further damage.

Employee injured left thumb while opening the rear axle cover on the Cat 1600 LHD. As they lifted the hood with right hand, left hand rested on top of the cab and as the cover passed the cab edge it pinched left thumb between the cover and the cab. EE was replacing a scoop push plate ram. Two miners were assisting employee. As the connection pin was being hit through the block and into the ram eye, the injured EE stuck a finger in the block hole to see if it was lined up. At that point the eyelet dropped a couple inches and the tip of the finger was pinched. This resulted in 11 stitches and the tip of finger fractured.

Operator was trying to remove a stuck bolt from a pan grind roller when a splinter of metal chipped off and penetrated the Operators left hand between EE's thumb and pointer finger. Operator had to receive medical attention to remove metal. Local anesthesia and tetanus shot given. No lost time, Operator returned to work.

Flash burns (welding)

Employee was helping a welder by holding the metal pieces while the welder tacked the pieces together. During the welding process, employee looked at the arch flash, which caused cornea flash burns to both eyes. Requested medical visit on 1/21/2021 and was prescribed medication for eyes.

Employee was attempting to clear clay grates with a steel bar. When a section of material broke loose, the employee's finger was pinched between the steel bar and steel clay grate. This required 6 sutures to close the wound.

I.P. was installing Insulated Metal Panels when I.P. lost their footing. As I.P. fell they grabbed on to the edge of the Panel that was going to be installed. The grabbing motion at the edge / corner of the panel tore I.P.'s glove and therefore cutting the right thumb inside the glove.

The employee was on the third rung of a 6'-0"" ladder using a harness with a retractable lanyard. The employee was tied off and using a plasma cutter to cut out grizzley bars. While cutting, slag went down the employee's shirt, and EE fell from the ladder. The platform the ladder was on had refractory on it, causing the employee to strain/sprain ankle when EE fell.

A maintenance laborer was grinding on a part which was secured in a vise in the shop. While grinding on the part, the grinding wheel came apart, throwing a piece of the wheel and hitting the left forearm of the employee causing a laceration to the arm. Five stitches were used to close the laceration.

Ee was working on a drive. Ee stood up, took a step back, and struck head on a bracket. Seen at the ER and laceration was closed with staples. Ee released to full duty 1/19/21.

Employee was in process of in-choking a chute. Employee was using an 8' air lance when they extracted the lance, the weight of the air hose and lance made it top heavy and the lance slipped from hands. the lance fell, striking them on the back of the head, causing a laceration, requiring 6 staples to close. Employee returned to work with no restrictions.

Employee was moving man basket with a forklift, the man basket base/weight was not secured with the rods that hold the basket to base. The basket tipped off of the base and fell forward toward the truck and hit a piece of pipe that was laid across the bed causing the pipe to flip up and hit EE on the right side of head & knee.

While using a grinder to remove a weld, a foreign object got on the employees left eye. EE later sought medical attention and had the object removed.

When employee was stepping away from the sump employee's right foot slipped causing employee to lose footing and fall on left side and strike left elbow off of the floor. Employee stated that they were helping an electrician fix a ride and they and another employee lifted the ride up. When they let it down employee got left thumb caught between the ride and a bottle jack resulting in 3 stitches.

The injured employee was working on an excavator bucket change in the upper shop yard. Icy spots were identified and a load of rock was spread to mitigate the hazard. Employee was walking to get a hammer when they slid on the ice under the rock and fell to the ground. Employee received a laceration to the right knee that required stitches.

Employee was changing a blower motor filter cover when EE twisted wrist and back. This was diagnosed as a simple sprain, and treated with first aid care for nearly 5 weeks (ice, ibuprofen). On February 24th, EE was given Physical and Occupational Therapy orders, making it a medical treatment case.

EE received laceration on Left Middle FInger while setting feeder floor on the primary crusher springs. One spring got caught up on feeder table, EE tried to line feeder table up with spring, in doing so the EE's hand got stuck between the spring and the feeder table causing a laceration on finger.

While maneuvering a screw tub into position so it could be bolted into place, the crew noticed a clevis that needed to be removed that was still attached to the end of the tub. While the EE was attempting to remove the clevis, the tub shifted and pinched the EE's right index finger between the side of the tub and the catwalk.

Employee was changing filter socks at end of shift. As EE raised the socks, EE felt pain in shoulder. Did not report for a week. Was seen by doctor on 1/20 and given an injection. X-rays showed no visible issues.

While working with a hammer on metal machinery parts, an airborne metal splinter lacerated the employee's cheek. First aid measures were applied but after a few days it was determined that a piece of the shard remained in the wound. On 1/20/21 an outpatient procedure was performed to extract the shard, and the employee was released to work without restrictions.
