#3 HOIST TRIPPED OUT AT 1600 MAINTENENACE CREW WAS ON BOARD. RESET/RESTORED POWER AND LOWERED CREW TO MINE LANDING. LOST DEPT INDICATOR. REPLACED THRU TROUBLE SHOOTING AVTRON PULSE GENERATOR. RETURNED TO SERVICE 9:00 AM DOWNTIME APPROX. 3 HOURS.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #3 ENTRY (RETURN) AT #6 CROSS CUT IN THE FIRST SOUTH, MAIN EAST - THIRD SOUTH PANEL. THE ROOF FALL WAS 80'LONG, 18'WIDE AND 8'HIGH. THE AREA WAS BOLT ED WITH 6'FULLY GROUTED ROOF BOLTS AND 2"" X 6"" X14'LONG BOARDS HAD BEEN INSTALLED IN THE IMMEDIATE AREA AS SUPPLEMENTAL SUPPORTS. THE ROOF MATERIAL CONSISTED OF LAMINATED SANDY DARK GRAY SHAL

#3 SHAFT MAN HOIST STOPPED AT 12:01 PM. AN ELEC-TRICAL SHORT WAS DETERMINED TO HAVE CAUSED THE PROBLEM. REPAIRS WERE MADE TO ALLOW THE XXX HOIST TO OPERATE. IT WAS RETURNED TO SVC.AT 1:30

TRUCK DRIVER WAS STOCKPILLING CRUSHED 1/4 INCH BASE MATERIAL. AS HE WAS ATTEMPTING TO DUMP THE 8TH LOAD OF THIS SHIFT THE TRUCK OVER TURNED ONTO IT LEFT SIDE. THE DRIVER WAS PINNED IN THE TRUC K BY THE STEERING WHEEL. LOCAL EMS WAS CONTACTED AND THE DRIVER WAS EXTRICATED WITHIN 25 TO 30 MIN. THE DRIVER DID NOT SUSTAIN INJURIES AS THE RESULT OF THE ACCIDENT.

ONE PIECE OF ROCK MEASURING ~10 1/2' LON G, 77' WIDE, AND FROM 29-12"" THICK CAME FROM THE LEFT RIB AND STRUCK THE VICTIM INFLICTING FATAL INJURIES.
VICTIM FELL FROM THE TOP FLOOR OF THE PREPARATION PLANT TO THE GROUND LEVEL APPROXIMATELY 101 FT. TO HIS DEATH.
THE VICTIM WAS IN THE PROCESS OF MOVING HIS JOY CONTINUOUS MINER FROM THE #3 ENTRY TO THE #4 ENTRY ON #3 UNIT WHENHE WAS CAUGHT BETWEEN THE RIB AND THE HEAD OF THE CONTINUOUS MINER WHICH INFLI CTED THE FATAL INJURIES.
2 RIGHT 5 SOUTH TRACK ENTRY, TWO EES WERE INSTALLING FISH PLATES IUNBY A FLAT CAR LOADED WITH RAILS AND ATTACHED TO A BATTERY LOCOMOTIVE. WHEN A THIRD EE ATTEMPTED TO NMOVE THE LOCOMOTIVE AND RAIL CAR OUTBY, IT TRAVELED INBY STRIKING THE EES INSTALLING THE FISH PLATES RESULTING IN FATAL INJURIES TO ONE EE AND MINOR CONTUSIONS TO THE OTHER EE. ACCIDENT IS STILL UNDER INVESTIGATION.
EMPLOYEE WAS USING A HIGH PRESSURE WATER HOSE TO WASH OUT ACCUMULATED MATERIAL BELOW CONVEYER #11 (FLOATING) AND TAILPULLEY CONVEYER #4 (LAND)APPARENTLY FELL IN WATER PIT WHILE NOT WEARING A LIFE JACKET.
WHILE PREPARING TO ATTACH ROCKDUST HOSE TO POD DUSTER, THE DUSTER WAS STRUCK BY 3 SUPPLY CARS WHICH HAD BROKEN LOOSE FROM THE SLOPE CAR AT THE TOP OF THE SLOPE. CARS STRUCK THE DUSTER & THE VI CTIM WAS FATALY INJURED. THE CARS TRAVELED APPROXIMATELY 1700 FT. BEFORE STRIKING THE ROCKDUSTER. CAUSE OF THE CARS BREAKING LOOSE IS STILL UNDETERMINED.
A ROOF FALL OCCURRED DURING MINING IN THE 5-LEFT WORKING SECTION WHILE CONNECTING THE XCUT BETWEEN NO. 7 AND NO. 8 ENTRIES. THE ROOF FALL RODE BACK THROUGHT THE INSTALLED ROOF BOLTS, STRIKING THE REMOTE CONTROLLED CONTINUOUS MINER OPERATOR AND MINER HELPER. THE MINER OPERATOR SUSTAINED SERIOUS PERSONAL INJURIES WHILE THE MINER HELPER SUSTAINED FATAL INJURIES.
DRIVER GOT OUT OF TRUCK, NOT SETTING PARK BRAKES, PULLED HAND BRAKE. WAS FOUND PASSENGER SIDE OF TRAILER WITH SERVICE LINE DISCONNECTED. DRIVER WAS LAYING ON GROUND HEAD TOWARD FRON OF TRUCK, TANDEMS HAD RUN OVER LEGS, STOMACH AND CHEST AREA. TRUCK WAS RUNNING AND HAD MOVED APPROXIMATELY 16' HITTING TRUCK IN FRONT.

HE CUT ONE SIDE FREE AND STARTED TO CUT THE OTHER SIDE WHEN THE PLATE GAVE WAY AND FELL ON TOP OF HIS RIGHT FOOT JUST BEHIND THE STEEL TOE AREA OF HISBOOT. THERE WAS A SMALL PUNCTURE WOUND AND A FRACTURE OF TWO BONES IN THE FOOT THAT REQUIRED SURGICAL REPAIR.

THE INJURED WAS WORKING IN THE CLEAN COAL TUNNELAT THE #4 PREP. PLANT. HE BENT OVER TO PICK UP A4' PIECE OF STEEL CHANNEL (WEIGHT 45-50LBS.) W/ HIS CO-WORKER WHEN HE FELT PAIN IN THE CENTER OF HIS LOWER BACK.
SUPERVISING MEN RE-RAILING AN EQUIPMENT DOLLY LOADED WITH A CONTINUOUS MINER. DOLLY WAS BLOCKED UP ON ONE SIDE WITH TWO WOOD CRIB BLOCKS. ATTEMPTING TO LIFT OTHER SIDE OF DOLLY WITH A SCOOP CA R. THE CRIB BLOCKS KICKED OUT AND ONE OF THEM STRUCK HIS KNEE.

VICTIM WAS OPERATING R3EMOTE CONTROL MINER WHEN HE WAS STRUCK BY A SHUTTLE CAR.
SNOWING AND RAIN. GROUND WAS SLICK AND ICY, TWISTED ANKLE, PULLED LIGAMENTS.
EE HAD JUST STARTED EATING LUNCH AND NOTICED AN UNUSUAL SMELL AND TASTE IN HIS SOUP AFTER TWO ORTHREE SPOONFULS. LATER THAT DAY EE WENT TO THE HOSPITAL FOR OBSERVATION. IT IS UNKNOWN AT THIS TIME HOW THIS INCIDENT OCCURRED. THIS INCIDENT IS UNDER INVESTIGATION BY THE SHERIFF'S DEPT. EE HAS NO ILL-EFFECTS FROM THIS INCIDENT.

EE AND EE HAD JUST LIFTED A 300 FT SECTION OF COILED WATER HOSE FROM THE BED OF A PICK UP TRUCK AND WAS UNWINDING IT ALONG SIDE AN OLD ROAD WHEN EE NOTICED A DULL PAIN IN HIS LOWER BACK. THE P AIN BECAME INCREASINGLY WORSE AS THE DAY PROGRESSED.
WHILE INSTALLING ROOF BOLT,HIS GLOVE GOT CAUGHT ON ROOF BOLT. TWISTED HAND AROUND BOLT.
THE INJURED EMPLOYEE WAS REACHING UNDER THE FRAMEWORK OF A USI CRUSHER TO TURN ON THE FUEL VALVE WHEN HE SLIPPED ON THE SNOW/ ICE COVERING THE GROUND. EMPLOYEE SAW A DOCTOR 5 DAYS LATER DUE TO PERSISTENT DISCOMFORT.
THE INJURED WAS WALKING OUT OF THE FILTER TRAILOR DOWN THE RAMP AND STEPPED ON A ROCK CAUSING HIM TO TRIP AND FALL.

A SHUTTLE CAR BROKE DOWN IN A ROADWAY AND WAS BEING PUSHED BACK WITH THE MINER. THE CAR SLID UP ON THE BUMPER OF THE MINER THEN FELL BACK OFFBOUNCING OPERATOR AGAINST CAB OF SHUTTLE CAR.
EE WAS WALKING OFF SECTION BEHIND ANOTHER EE WHOWAS CARRYING A BELT CUTTING TOOL OVER HIS RIGHT SHOULDER. THE TOOL SNAGGED A PIECE OF PHONE WIREHANGING FROM A PIECE OF CABLE. THE WIRE FLEW BAC K AND HIT HIS RIGHT EYE.

EE WAS WALKING ACROSS SECTION AND SLIPPED, TWISTING HIS RIGHT KNEE. HE RETURNED TO WORK HIS NEXTSCHEDULED SHIFT BUT AN MRI REVEALED CARTILAGE DAMAGE TO HIS KNEE. ON JAN 29, 2002 HE WENT BACK T O DOCTOR OVER SWELLING TO RIGHT KNEE. DOCTOR ORDERED HIM OFF FOR 2 WEEKS DUE TO MORE TEARING OF CARTILAGE.
EMPLOYEE WAS BACKING OUT FROM THE EAST SIDE OF THE BUNKER AT THE SAME TIME AS THE WATER TRUCK WAS BACKING OUT THE CROSSCUT ACROSS FROM THE PV. THE WATER TRUCK HIT THE PV EMPLOYEE WAS RIDING IN. LOST TIME INJURY.
EE HAD BEEN WELDING CUTTING EDGES ONTO AN ENDLOADER BUCKET. HE FELT AN IRRITATION IN HIS RIGHT EYE AND RUBBED HIS EYE WITH HIS GLOVES HAND CONTAMINATED WELDING GLOVE. THIS CAUSED HIS EYE TO BE COME FURTHER IRRITATED WHEN A FOREIGN OBJECT GOTINTO THE EYE, PROBABLY OFF OF THE GLOVE. INJURED AND ANOTHER EE WERE CARRYING A STEEL STRAP TO THE BAND SAW, WHEN HE NOTICED PAIN IN HIS BACK.
EE STATED THAT HE WAS CLIMBING ONTO A DOZER TO ADD HYDRUALIC OIL TO THE DOZER. HE HAD THE OIL HOSE IN HIS L. HAND W/ HIS R. HAND ON THE HAND-RAIL USING IT TO PULL HIMSELF UPON THE DOZER. DUR ING THIS PROCESS, HE FELT A MUSCLE PULL IN HIS LOWER RIGHT SIDE. INJURY NOT REPORTABLE UNTIL INCURRING LOST TIME ON 11-29-01.
EE WAS STACKING GLUE INTO GLUE RACK ON BACK OF #31-08 SECOMA PINNING RIG WHEN HIS FOOT SLIPPED ON SURFACE OF PINNING RIG CAUSING HIM TO FALL TO GROUND LANDING ON HIS FEET. EE WAS TAKEN TO LOCA L HOSPITAL AND XRAY REVEALED A FRACTURE TO THE DISTAL END OF THE LEFT LEG FIBULA.

THE EMPLOYEE STEPPED OVER THE BERM WITH HIS RIGHT FOOT CONTACTING EXPOSED PLASTIC SHEETING. AS EMPLOYEE'S WEIGHT SHIFTED THE RIGHT FOOT SLIPPED ON THE PLASTIC CATCHING ON UNEVEN SURFACE CAUSING ANKLE TO TURN INWARD.

HE WORKED THE BALANCE OF THE SHIF T WITH THE ARM NOT HURTING BUT STATED WHEN HE GOT HOME THE ELBOW STARTED SWELLING HE REPORTED THE INJURY ON 11-26-01. EE DIABETIC STARTED MISSED WORK ON 11-27-01.
EMPLOYEE STATED HE WAS OPERATING CONTINUOUS MINER, TOP BUMPED, PIECE OF ROCK FELL STRIKING HIM ON THE LEG.
INDIVIDUAL'S BACK BEGAN HURTING UPON TURNING TO LOOK BACKWARDS WHILE BACKING UP A PICKUP TRUCK. THERE WAS NO MECHANISM OF INJURY AND INDIVIDUAL HAS A HISTORY OF BACK PROBLEMS. THEREFORE, THIS IS BEING CONSIDERED AN OCCUPATIONAL ILLNESS.
HE WAS COMING DOWN OFF THE MOUNTAIN. HE STEPPED FROM ONE ROCK TO ANOTHER THAT HAD MOSS ON IT. HE SLIPPED, FALLING DOWN ONTO HIS KNEES AND ELBOWS. RT. KNEE APPARENTLY HIT FIRST.

EE WAS USING A RATCHET AND A SOCKET TO LOOSEN BOLTS THAT WERE BADLY CORRODED. AS HE PULLED ON THE RATCHET, HE FELT SOMETHING ""POP"" IN HIS SHOULDER. THERE WAS NO DAMAGE TO EQUIPMENT OR THE MINI NG OPERATION. EE NEEDED TO USE A BREAKER BAR TO LOOSEN THE BOLTS. NO OTHER FACTORS WERE INVOLVED.
EE WAS TRYING TO LIFT A ROCK OFF OF THE MINER. A KNOT CAME UP IN HIS LOWER RT. ABDOMEN.
EE STATED HE ATTEMPTED TO TIGHTEN A NUT ON A SHAFT OF PICK BREAKER WHEN THE FLASH BOARD MOVEDBACK CATCHING THE BOTTOM PORTION OF HIS R. INDEXFINGER BETWEEN THE SPLASH BOARD AND THE NUT.
EE INJURED SHOULDER WHILE SWINGING SLEDGE HAMMER& HANDLING HEAVY METAL. THIS IS AN OLD INJURY FROM 5/10/01 THAT HE AGGRAVATED.
EE WAS SHOVELING UNDER THE CONVEYOR BELT WHEN HE STRAINED HIS LOWER BACK. HE HAD PAIN IN HIS BACK AND LEFT LEG.
EE WAS ATTEMPTING TO PUT SUPPLY CAR ON TRACK W/ AN AIR BAG. WHILE BLOWING UP THE AIRBAG A PIECE OF WOOD FLEW OUT STRIKING HIM ON THE LEFT SIDE OF THE FACE. RESULTING IN CUT ABOVE THE L. EYE & BONE FRACTURE OF LEFT SIDE OF FACE.
THIS IS A RE-INJURY OF AN ACCIDENT THAT WAS MEDICAL ONLY. EE HAS SINCE HAD OUT-PATIENT SURGERY TO REPAIR KNEE. EE WAS INSTALLING BREAST PLATE IN SECONDARY CRUSHER, STANDING ON V-BELT GUARD AND STEPPED DOWN ONTO COUPLING GUARD. THE GUARD SHIFTED CAUSING EE TO LOSE BALANCE AND FALL.
EMPLOYEE STATED HE WAS BUILDING A BRATTICE, CARRYING AN OMEGA BLOCK AND STEPPED ON A PALLET. HIS FOOT WENT BETWEEN BOARDS ON PALLET CAUSING HIM TO TWIST HIS ANKLE AND FALL. DID NOT MISS WORK U NTIL 1/7/02 AND THEN HE STATED DOCTOR SAID HE HAD BONE SPURS SO IT WAS NOT RELATED TO AN INJURY UNTIL WE RECEIVED A DOCTOR'S REPORT RELATING IT TO THE ABOVE INJURY DATE.
EE WAS CLOSING DOORS ON RAILROAD CARS. HE BENT OVER TO CLOSE THE LATCH ON THE RAILROAD CAR DOOR AND AS HE ROSE BACK UP HE FELT PAIN IN HIS LOWERBACK AREA. WENT TO DOCTOR & MISSED 1 DAY'S WORK.
EMPLOYEE WAS CLEANING (SHOVELING, SWEEPING) AT A BIN WHERE CEMENT DUST WAS PREVALENT.

WHILE EMPLOYEE WAS STANDING ON THE TOP STEP OF CAT OF THE MOUNTING LADDER, AFTER COMPLETED CONVERSATION WITH THE OPERATOR, THE OPERATOR NOTICED THE EMPLOYEE HAD GONE ASHEN AND HIS EYES WERE BEGINING TO ROLL BACK AND NO RESPONSE, HE BEGAN TO FALL OFF THE LADDER AND THE OPERATOR WAS UNABLE TO KEEP HOLD OF HIM. HE FELL DOWN THE SIDE AND HIT GROUND WITH LEFT SHOULDER.
HE WAS IN A GRADER BACKING UP AND HIT A ROCK AND HE SAID HIS HEAD HAD HIT THE GLASS BUT HE KEPT WORKING. THEN AT 10:30 AM HE WAS GRADING THE ROAD & HIT ANOTHER ROCK JOLTING HIS NECK. HE LEFT T HE JOB & WENT HOME. HE CALLED THE OFFICE ON NOV 20 AND SAID HE WAS OK. THEN ON NOV 21 HE CALLED THE OFFICE & WENT TO FAMILY DOCTOR.
EE FAILED TO COME TO A COMPLETE STOP AND YEILD RIGHT OF WAY TO THE COAL HAULER AT THE MAIN MINEINTERSECTION. CONTRIBUTING FACTOR, WINDOWS ON EE'S VEHICLE WERE ICED (FOGGED) UP. EE'S VEHICLE WAS TOTALED
EE EXITED THE SHOVEL CQAB TO THE WALKWAY WHEN THE SHOVEL BEGAN MOVING. HE TURNED AND WAS ENTERING THE CAB AGAIN WHEN THE SHOVEL TRAMMED OVER A ROCK CAUSING THE SHOVEL CAB DOOR TO SLAM SHUT ON HIS THUMB.

HIS LEFT HAND SLID DOWN THE WEDGE CAUSING A WOOD SPLINTER TO LODGE IN HI S MIDDLE FINGER OF HIS LEFT HAND. HIS HAND WAS BOTHERING IN DECEMBER AND HE REPEATEDLY WENT TO THE DOCTOR TO SEE THE CAUSE OF A INFECTION. DOCTOR FOUND SOME SPLINTERS IN HIS FINGER.
EE WAS GETTING OFF OF THE D-5 DOZER, WHEN HE SLIPPED ON THE TRACK, HOLDING ON TO THE HAND RAIL, PULLING HIS SHOULDER OUT OF PLACE.
EE HANDED THE SLEDGE HAMMER ACROSS THE BOOM OF THE CONTINUOUS MINER TO A CO WORKER. A PIECE OF MINE ROOF FELL MASHING HIS ARM AGAINST THE BOOM.
WAS LIFTING A BAG OF PUMICE AND PUTTING IT DOWN AND FELT PAIN IN GROIN, RESULTING IN A HERNIA.
WHILE OPERATING THE LONGWALL SHEARER, EE WAS KNEELING IN A LOW CONDITION WITH HIS HAND RESTING ON THE FACE CONVEYOR HAND RAIL. THE SHEARER DRUM KICKED UP A ROCK WHICH STRUCK HIS HAND RESULTING IN A FRACTURE TO THE HAND.
EMPLOYEE WAS CLOSING GATE, TRUCK ROLLED BACKWARD CATCHING EMPLOYEE BETWEEN TRUCK BUMPER AND GATE RESULTING IN INJURY.
EE STEPPED DOWN FROM KILN BEARING ONTO WATER PIPES AND TWISTED ANKLE.
DRAW ROCK FELL WHILE EMPLOYEE WAS REMOVING A DRILL STEEL FROM THE HOLE. THE DRAW ROCK STRUCK EMPLOYEE IN HIS LEFT HAND.
RIDING SCOOP TO END OF TRACK FROM SECTION THE SCOOP RAN OVER A ROCK IN THE ROADWAY AND BOUNCED THE EMPLOYEE. HE THEN SAID HIS BACK STARTED TO HURT.

INJURED WAS DESCENDING FROM QUARRY HAUL TRUCK WHEN HE SLIPPED STRAINING HIS BACK. DID NOT SEEKMEDICAL ATTENTION UNTIL 11-19-01, AT WHICH TIME HE WAS DISABLED BYU COMPANY DOCTOR.
EE WAS LIFTING OXYGEN TANK FROM JEEP WHEN HE TWISTED LOWER BACK AND FELT PAIN. *LOST WORK DAYS BEGAN 11-12-2001.
EMPLOYEE WAS STANDING ON A CLEAN COAL BELT ON THE FIRST FLOOR OF THE PLANT APPROX. 30"" ABOVE THE GROUND CUTTING A 4"" PIPE. THE PIPE FELL ON HIS TORCH HOSES AND CUT THEM. THEY BEGAN BURNING AND HE JUMPED OFF THE BELT TO CUT OFF THE TORCH VALVES, TWISTING HIS ANKLE. DID NOT GO TO DOCTOR OR LOSE TIME FROM WORK UNTIL 12/30/01.
EMPLOYEE WAS BUILDING A CRIB AFTER A SHIELD HAD BEEN PULLED AND RECOVERED. A CRIB BLOCK WAS PASSED INTO THE EMPLOYEE. THE CRIB STRUCK THE INJUR-ED EMPLOYEE ON THE RIGHT HAND.
EMPLOYEE WAS WALKING DOWN STAIRS WHEN HE SLIPPED AND FELL BACK. HE TRIED TO CATCH HIMSELF AND LANDED ON HIS LEFT WRIST. NOTED TO EMPLOYEE TO SLOWDOWN GOING DOWN THE STAIRS.
EE GOT BETWEEN 270 TRACTOR AND FEEDER WHILE HOOKING UP A CHAIN. THE TRACTOR JUMPED ON POINT CATCHING HIM BETWEEN THE FEEDER AND TRACTOR.

EMPLOYEE WAS USING A PRY BAR IN THE QUARRY ON LEDGE #1 AS HE WAS PRYING HE FELT A PAIN IN HIS MIDDLE BACK.

WHILE BOLTING TOP A PIECE OF DRAW ROCK FELL ON THE EMPLOYEE'S BACK, BREAKING A RIB.
VICTIM WAS SITTING ON LAUNCH VEHICLE DECK. AN AIR LINE BLEW APART AT THE CABLE REEL ON THE UP-PER PLATFORM. IT STARTLED HIM AND HE JUMPED OFF THE LAUNCH VEHICLE. THE EE WERE REMOVING OLD RAIL. THEY HAD HOOKED UP TO A SECTION OF RAIL. THE WEB SLINGS WHERE HOOKED TO THE END OF EACH RAIL AND CONNECTED TO THE CENTER OF THE LHD BUCKET. THE OPERATOR STARTED TO PULL THE SECTION OF RAIL OUT. THE LEFT WEB SLING BROKE CAUSED THE RAIL SECTION MOVE TOO THE LEFT STRIKING THE EE.
ADVISED FOREMAN THAT WHILE HANDLING MINER CABLE HE FELT PAIN IN HIS GROIN. INITIAL DOCTOR VISIT DIAGNOSED SPRAIN/STRAIN. ON 12/26/01 DOCTOR ADVISED HE NEEDED HERNIA REPAIR. ON 02/18/02 HE WAS CLEARED TO RETURN TO FULL DUTY.

EE WAS PREPARING TO LOAD A TRUCK. HE HAD THE FORKS TOO HIGH, WHICH CAUSED THE PORTABLE LOADING PLANK TO TIP OVER. HE WAS STRUCK BY THE PLANK ON THE LEFT. HE GOT OFF OF THE FORK TO TRY TO LINE UP THE PLANT BETWEEN THE BUILDING AND THE TRUCK. THAT IS WHEN IT TIPPED OVER AND STRUCK HIS RIGHT LEG CAUSING A CONTUSION AND A CUT.
A GRAVELED PARKING LOT WHERE THE EQUIPMENT IS PARKED OR SERVICED. EE WAS RETURNING BACKHOE TO LOT AND LOST CONTROL AND RAN INTO A DODGE RAM TRUCK. HIT HIS SHOULDER ON THE STEERING WHEEL.
EMPLOYEE WAS STEPPING DOWN ON PUSH ARM OF DOZER WHEN HE TWISTED AND FELT HIS RIGHT KNEE POP.
EE WAS WORKING ON SPEED REDUCER ON #1 CLINKER ELEVATOR AND FELL THROUGH A HOLE IN THE FLOOR.

ON WED. NOV. 7TH, 2001 @ 2PM, EMPLOYEE WAS OPER-ATING THE JAW CRUSHER, BREAKING A JAM WITH A STEEL WEDGE ATTACHED TO A 1/2"" STEEL CABLE. THE EYE BROKE OFF OF THE WEDGE ALLOWING THE WEDGE TO DROP INTO THE JAW. THE WEDGE THEN SHOT UP OUT OFTHE JAW STRIKING EMPLOYEE ON THE RIGHT ARM.
EMPLOYEE'S SHIFT WAS OVER. HE REACHED INSIDE HIS TRUCK TO START IT. HIS TRUCK WAS IN GEAR AND DRAGGED HIM ACROSS THE PARKING AREA IN BETWEEN 2 OTHER TRUCKS. THIS CRUSHED THE DRIVERS DOOR AROUN T HIS UPPER BODY. THEN HIS TRUCK RAN OVER HIS LEG.
STEPPING OUT OF ARTICULATED TRUCK ONTO THE LADDER. HE BENT HIS KNEE THE WRONG WAY AND STRAINED KNEE. HE WORKED THE REMAINDER OF THE DAY AND ALSO THURSDAY AND FRIDAY BUT CONTINUED TO HAVE PAIN. HE WENT TO THE DOCTOR 11/13/01 AND REPORTED THE ACCIDENT. HE WAS OFF WORK THAT DAY AND 4 1/2 DAYS AFTER THAT FOR THERAPY.
EE WAS LIFTING A PUMP & TWISTED RESULTING IN LOWER BACK PAIN. ON 11/13 EE REPORTED THAT HIS BACKWAS STILL HURTING. A DOCTOR'S APPOINTMENT ON 11/14 WAS SECURED & EE WAS PLACED ON LTA STATUS.
THE EE, A MOBILE EQUIPMENT REPAIRMAN, WAS CLOSING A HOOD LATCH ON THE CAT GRADER. THE ENGINE WAS RUNNING TO ALLOW FOR EVALUATION OF AN ANTIFREEZE LEAK WHEN TWO FAN BLADES CUT THE INSIDE OF HIS LEFT ARM.

MEMBER WAS GOING TO ANOTHER SPREAD OF EQUIPMENT TO FUEL, WHEN GOING DOWN A HILL HE LOST CONTROL OF THE TRUCK CAUSING HIM TO HIT A MOUNTAIN SIDE.THIS RESULTED IN A CRACKED PELVIS AND CHEST BRUISES.

SLIDING ONE ROCK OFF OF ANOTHER ROCK, WORKER GOT HIS FINGER TIP PINCHED BETWEEN THE 2 ROCKS.
INJURED'S FINGER WAS CAUGHT IN THE CLOSING ELEVATOR DOORS.
EE WAS CLIMBING UP LADDER ON 992C LOADER. LADDER HAD MORNING DEW ON IT CAUSING HIS FOOT TO SLIP AND FALL TO GROUND FROM APPROX. 10' (SECOND RUNG FROM THE TOP).
ON 11-6-01 EE STATED THAT HE WAS DRIVING OUT OF THE PIT WHEN A CALL CAME IN ON THE RADIO. AS HE REACHED OVER TO PICK UP THE RADIO HANDSET HE TOOK HIS EYE OFF THE ROAD AND DROVE INTO THE ROAD B ERM. HE STATES THAT HE THINKS HE WAS KNOCKED UNCONSCIOUS MOMENTARITY AND FELL OFF THE DRIVER SEAT. HE SUBSEQUENTLY GOT BACK ONT HE SEAT AND BROUGHT THE VEHICLE TO A STOP. LATER THAT DAY HE WE
ATTEMPTED TO GET OUT OF SERVICE TRUCK & TWISTED HIS LOWER BACK.
THREE EE WERE GROUTING PILLARS IN A-4 PILLAR AREA, WHEN THE 3/4"" GROUT HOSE BECAME PLUGGED WITH HARD CEMENT. THE HOSE BLEW APART INT HE EE'S RIGHT HAND SPRAYING CEMENT IN HIS RIGHT AND LEFT EY ES. EE WAS TAKEN TO DR WHERE THEY WASHED OUT HISEYES DR REFERRED EE TO DR AN OPTOMETRIST IN FARMINGTON. EE WAS DIAGNOSED WITH SEVERE IRRITATION TO BOTH EYES. WHICH WERE BANDAGED A FOLLOW UP V
