HOIST OUTAGE EXCESS 30 MINUTES WIRE BROKEN ON HOIST CONTROLLER DOWN 9:30 PM UP 11:20 PM.
EAST 3 9 BLOCK #8 HEADING 4-WAY INTERSECTION ON MMU 008. 7 FT HIGH X 18 FT WIDE X 20 FT LONG. TYPE OF SUPPORT 6 FT RESIN BOLTS, 6"" X 16"" PLATES, ROOF MATS. FALL DANGERED OFF ADDITIONAL ROOF SU PPORTS SET AROUND FALL.
WHILE HOISTING ORE FROM THE MINE TO THE SURFACE, THE HOIST WOULD NOT MOVE AFTER THE SKIP WAS FILLED. AN IMMEDIATE INVESTIGATION REVEALED A PROBLEM WITH THE AIR PRESSURE AND THE BRAKE WOULD NOT RELEASE. THE HOIST OPERATOR CALLED AN ELECTRICIAN, THEN CALLED THE SHIFT FOREMAN ON DUTY IN THE MINE. ALL PRODUCTION IN THE MINE HAD ALREADY STOPPED AS THE CREWS WERE MAKING SHIFT CHANGE.
ROOF FALL DISCOVERED WHILE INSPECTING ROOF FALL AT MOUTH OF 1 LEFT PANEL. THIS FALL WAS IN #3 ENTRY 2ND OPEN BREAK OUTBY INACTIVE 1 LEFT MAINS FACE AT SURVEY SPAD 1090. SECTION WAS MOVED OUTBY THIS FALL ALSO BEFORE MINING RESUMED.
AROUND 10AM IT WAS REPORTED THAT THERE WAS ""SMOKING"" COAL AT THE BOTTOM OF COAL SILO. HOT ENOUGH TO BE MELTING FEEDER BELT. WE IMMEDIATELYSTARTED PUTTING WATER ON AT THE TOP OF THE SILO AND AT THE BOTTOM. WE OPENED THE EMERGENCY DUMP CHUTE AND EMPTIED APPROX. 400 TONS OF COAL ONTO THE GROUND. ONLY DAMAGE WAS TO FEEDER BELT. NO INJURIES.
A BROKEN WELD ON THE BOTTOM OF THE SKIP RESULTEDIN THE SKIP BEING REMOVAL FROM SERVICE. THE MINEWAS EVACUATED AND THE SKIP WAS REPAIRED AND TESTED BEFORE MINERS RE-ENTERED THE MINE.
A ROOF FALL OCCURRED IN THE #2 ENTRY OF UNIT 2 MMU 003-0 AT THE 8 +45' STATION. LENGTH 60' WIDTH 17' HEIGHT 7 TO 9'.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE INTERSECTION OF BREAK #15, 1 BREAK OUTBY SPAD #2087 IN THE #2 ENTRY ON #8 MAINS. THE FALL WAS ESTIMATED TO BE 18'W X 20'L X 5'H. AREA HAD BEEN SUPPOR TED WITH 48"" FULLY GROUTED RESIN BOLTS. NO INJURIES, NO EQUIPMENT CAUGHT, VENTILATION WAS NOT INTERRUPTED.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE WEEKLY EXAMINER IN THE B-1 PANEL, NO. 8 ENTRY 10 FT INBY SPAD 3844. THE FALL MEASURED 35 FT LONG, 17 FT WIDE AND 8 FT HIGH.
A ROOF FALL WAS FOUND BY THE 2NE SHIFT MINE FOREMAN WHILE PRESHIFTING THIS MINE. THE ROOF FALL WAS IN THE #1 ENTRY IN THE 2ND SOUTH PANEL AT SPAD #5+90. THE FALL WAS ABOUT 50' LONG, 18' WIDE & 4' HIGH. 41"" FULLY GROUTED ROOF BOLTS WAS USED IN THE AREA.
A ROOF FALL MEASURING ~20' IN LENGTH, 18' WIDE &8' TO 10' THICK WAS FOUND LOCATED IN THE #1 ENTRY 10TH CROSSCUT ON THE NO. 3 BELTLINE MAINS.
AN UNINTENTIONAL ROOF FALL OCCURRED AT 4 AND 80 ENTRY #1 IN THE 6TH NORTH INTAKE THE FALL DID NOT IMPEDE TRAVEL OR IMPAIR VENTILATION. THE FALL MEASURED 30' LONG 19' WIDE AND 3' THICK. THE FAL L DID NOT DAMAGE ANY EQUIPMENT. THE TOP WAS SUPPORTED BY 4' FULLY GROUTED ROOF BOLTS. THE 3 WAY INTERSECTION WAS DEVELOPED 12-2000.
THE FRONT BEARING ON THE SERVICE HOIST MG SET FAILED. TWO EMPLOYEES WERE UNDERGROUND AT THIS TIME, AND WERE EVACUATED BY OUT NO. 2 MINE SHAFT SECOND WAY OUT. THE MG SET BEARING WAS REPLACED AN D HOIST BACK IN OPERATION ON 11/01/01 AT 2:00 PM.
A ROOF FALL OCCURRED IN THE 8TH EAST, #2 ENTRY. BETWEEN #17 AND #18 CROSSCUTS. THE FALL WAS 40'L X 18'W AND 8' THICK. THE FALL DID NOT AFFECT TRAVEL, VENTILATION, DAMAGE EQUIPMENT OR CAUSE ANY INJURY THE AREA HAD BEEN BOLTED WITH 6' POINT ANCHOR BOLTS.

AT APPROX 1150 ON 10-29, A CONTRACTOR EE NOTICED SMOKE IN AREA B. UPON INVESTIGATING, HE FOUND DFIRE BURNING OUT OF CONTROL. HE NOTIFIED THE CO. WHO CONTACTED LOCAL FIRE DEPTS. TO RESPOND. AT APPROX 1345, THE FIRE WAS CONTAINED ON THE RIGHT-OF-WAY. A PIPELINE CLEARING CONTRACTOR HAD BEEN ATTENDING BRUSH PILE FIRES ON THE RIGHT-OF-WAY WHEN THE FIRES GOT OUT OF CONTROL.

A METHANE IGNITION OCCURRED IN THE FACE OF THE #3 ENTRY OF MAINS SECTION OF THE MINE. THERE WERENO INJURIES OR PROPRTY DAMAGE. FIRE WAS EXTINGUISHED BY ROCK DUST.

ROOF FALL IN XC 4+90 BETWEEN #2 & #3 ENTRIES (MMU-002) 3RD SOUTH OFF 3RD WEST. THIS WAS A HUGE SLIP RUNNING ALL THE WAY THRU THIS XC FROM #1 TO #5 ENTRIES. SIZE 18' X 40' X 10' ABOVE COAL. ARE A MINED 1 WK OCTOBER 2001.
ROOF FALL #8 ENTRY TRAVELWAY 99 XCUT MAIN NORTH 4 WAY INTERSECTION 20' LONG 25' WIDE 5' HIGH RE-ESTABLISHED SECONDARY ESCAPEWAY.
WE HAD A FALL 1 BREAK INBY SPAD #827 IN THE NW MAINS. IT MEASURED 35' L X 18' W X 4-6' T, AND COVERED NO. 2 BELT LINE AT THIS POINT. FALL WAS CLEANED UP AND FEDERAL PERSONNEL INVESTIGATED THE AREA ON 10-23-01.
ROOF FALL IN NORTH MAINS. SHALE SEPARATED FROM SANDROCK. OCCURRED AT THE END OF NORTH MAIN FROM #4 ENTRY TO #5 ENTRY. FALL WAS APPROX, 68' X 18' X 12'H. AREA WAS DANGERED OFF.
A ROOF FALL OCCURRED IN THE #9 ENTRY 3 WAY INTERSECTION RETURN #2 UNIT 1 X-CUT OUT BY SPAD #2652L 23' BY W 18' BY H 12'. THE ROOF WAS BOLTED WITH 6' POINT ANCHOR AND 12' CABLE BOLTS.
A ROOF FALL OCCURRED 50' TO THE LEFT OF SURVEY STATION NO. 7535 ON THE NO. 6 RIGHT SIDE POWER ENTRY BEING 1 CROSSCUT INBY THE POWER CENTER. NO PERSONAL INJURIES WERE REPORTED AS A RESULT OF TH IS FALL.
HAUL TRUCK PULLED UP TO CONVEYOR AND LOADER OPERATOR BACKED UP AND RAN INTO HIM. IT DENTED THE CORNER OF THE BUMPER OF THE HAUL TRUCK. THERE WERE NO INJURIES TO EITHER OPERATOR.
A FIRE STARTED ON BELT TIRE OF LORC-2. CONVEYOR WAS TOTALLY DESTROYED AND DAMAGE OCCURRED IN TOP TWO FLOORS OF BATCH LOADOUT BUILDING. AFTER INVESTIGATION, WE FEEL A PEN THAT WAS COVERED AND HIDDEN, AND NO ONE WAS AWARE OF ACCUMULATED COAL FINES AND STARTED FIRE AT WEIGHT TRANSFER TOWER.
A FALL WAS DISCOVERED IN THE 3RD NORTHWEST PANEL E1 AND E2 OF 7 AND 50 AREA WAS MINED IN MAY-JUNE 2001. THE FALL MEASURED 66 L 17 W 6 1/2 H. FALL INVESTIGATED BY MSHA.
A ROOF FALL ON THE 003 SECTION AT #29 CROSSCUT ROOM LEFT OFF #1 ENTRY AT SPAD 10105. THE FALL WAS APPROX 50' LONG 6-10' HIGH AND 18' WIDE. THE FALL CAUGHT THE CONTINUOUS MINER AND SHUTTLE CAR , NO INJURIES.

NORTH MAINS 51 BLOCK #5 HEADING, 4-WAY INTERSECTION MMU 003 14 FT HIGH BY 18 FT WIDE BY 20 FT LONG. TYPE OF SUPPORT, 6 FT RESIN, 6"" BY 16"" PLATES. ROOF MATS, SCREEN ACS JACKS FALL DANGERED OFF . ADDITIONAL ROOF SUPPORT SET AROUND FALL. FALL CLEAN UP PLAN DEVELOPED AND POSTED AT FALL.
SHEARER OPERATOR WAS MINING TOWARDS THE TAILGATE AT 99 SHIELD. HE OVBSERVED AN ORANGE FLAME THAT LASTED ~2-3 SECONDS AND SELF EXTINGUISHED.
THERE WAS A PROBLEM WITH AN ELECTRICAL SWITCH TO THE BREAK SYSTEM. ALL OTHER HOIST WERE TO BREAK WITH WAS CLEANED AND ELECTRICITY RESTORED.
MALFUNCTION OF HOIST EQUIPMENT. THE MOTOR SHAFT TACOMETER BRACKETT LOOSENED, SLIPPED ON SHAFT CAUSING MALFUNCTION OF HOISTING CONTROLS. HOISTINGEQUIPMENT OUT OF SERVICE FROM 11:45 TO 12:57PM.
THERE WAS A PROBLEM WITH AN ELECTRICAL SWITCH TOTHE BREAK SYSTEM. ALL OTHER HOISTS WERE OPERATIONAL. THE BREAK SWITCH WAS CLEANED AND ELECTRICITY RESTORED.

NO ONE SAW THE ACCIDENT BUT A SHOVEL WAS FOUND BY THE BODY IN THE CONE FEED BELT, AND THE VICTIM WAS FOUND IN THE TENSION PULLEY FOR THE UNDERCONE CONVEYER BELT. IT APPEARS THAT HE LOST HIS BALANCE OR TRIPPED AND FELL INTO THE MOVING BELT.

EE WAS OPERATING A DM600 MACK FUEL TRUCK WHEN HE APPARENTLY LOST CONTROL OF THE TRUCK AND JUMPED OUT OF THE TRUCK. IN DOING SO, HE SUSTAINED SEVERE HEAD INJURIES AND DIED ON 10-4-01. THE ACCID ENT HAPPENED ON A DESCENDING ROAD. CAUSE IS STILL UNDETERMINED.
INDIVIDUAL CAME INTO CONTACT WITH OVERHEAD POWERLINE CAUSING DEATH BY ELECTRICUTION.
IN THE PROCESS OF SETTING FEEDER ON TAIL PIECE, THE CRIB BLOCKS BROKE LOOSE WHICH ALLOWED THE FRONT OF THE FEEDER TO DROP DOWN CRUSHING HIS RIGHT THUMB.

AS THEY WERE TRYING TO POSITION CANOPY LEGSOVER SOCKETS THE CANOPY SLIPPED AND CAUGHT EE LEFT HAND BETWEEN CANOPY LEG AND DECK OF UNI-TRAC.
AS INJURED EE WAS REMOVING A CANOPY FROM THE ROOF BOLTER, A POST ON THE CANOPY FELL OVER STRIKING THE EE RIGHT THUMB. ABOUT 1/2 INCH OF THE THUMB WAS AMPUTATED.
EE WAS CUTTING LOOSE A PIECE OF EQUIPMENT, HE DID NOT GET IT CUT COMPLETELY AND WHEN THE CRANE TRIED TO LIFT IT, THE PIECE BROKE LOOSE AND CAUGHT HIS FINGER.
MINER WAS ATTEMPTING TO SLIDE A SHIM INTO PLACE ON THE CONE CRUSHER WHEN HIS THUMB WAS MASHED BETWEEN THE SHIM AND THE UPPER BONNET OF THE CRUSHER.
SETTING IN BOLTER OPERATORS COMPARTMENT BACKING ROOF BOLTER OUT OF A CUT HAD LEFT HAND ON EDGE OF CANOPY AND HIT THE RIB. SEVERING FINGER.
EE WAS DIRECTING A LOADER TO LIFT A GRIZZLY SCREEN TO REPLACE SPRINGS. THE GRIZZLY SLIPPED OFF THE LOADER DUCKED AND THE SPRING HOUSING PINCHED OFF THE END OF THE EE'S LEFT THUMB. EE WAS WEARI NG GLOVES AT THE TIME OF THE INCIDENT.
THE EMPLOYEE WAS ADJUSTING A BELT ON THE VIBRATING SCREENS AND HIS FINGER WAS CAUGHT BETWEEN THE BELT AND THE PULLEY AND THE TIP OF HIS FINGER WAS CUT OFF.
WHILE IN THE PROCESS OF PICKING UP AND MOVING 2-12' STEEL BOOMS, EE CAUGHT HIS RIGHT INDEX FINGER BETWEEN THE 2 BOOMS. THE RESULTANT INJURY WAS AN AMPUTATION TO THE TIP OF HIS FINGER. EE FAILED DRUG TEST.

INDIVIDUAL HAD PLACED PUSHER STEEL ON FINISHED AND WAS RAISING HEAD UP ONTO STERIS. PUSHER FOUNED IN CHUCK CAUSING STEELS TO FLEX BOW THEN THE PUSHER SPRANG INTO BOTTOM OF THE CHUCK. INDIVIDUA L HAND WAS HOLDING STEEL WHEN IT FLEXED BACK STRAIGHT STIRKING THUMB.
EE HAD CLIMBED UP ON AN I BEAM ABOUT 20' FROM THE GROUND TO GREASE #6 HEAD ROLLER AND FELL OFF OF THE I BEAM.
ON 11/03/01 EMPLOYEE REPORTED THAT HE WAS HAVINGPAIN IN HIP AND L.POB WANTING TO SEE A DR.DENIEDWORK INJURY ONE SUPERVISOR DOCUMENTED EMPLOYER ILLNESS. THE PLANT WAS SHUT DOWN. THE EMPLOYEE WAS REMOVING A BOLT TO REMOVE A GUARD WHEN HE LIFTED HIS HEAD UP STRIKING HIS HEAD ON THE BOTTOM LEFT CORNER OF THE GUARD RESULTING IN THE LACERATION.
ATTEMPTING TO FREE A STUCK DRIVE CHAIN ON A SCOOP CAR WINCH. THE CHAIN BROKE AND STRUCK HIS LEFTINDEX FINGER.
AFTER HEARING UNUSUALY NOISE, EE LEFT BUILDING, WALKED OUT ONTO CATWALK TO OBSERVE AC CRUSHER. AFTER SEEING NOTHING WRONG, TURNED TO WALK BACK TO CONTROL BUILDING AND WAS STRUCK JUST ABOVE RIG HT EYE BY A ROCK, THAT APPARENTLY EJECTED FROM CRUSHER TRAVELING 15' UP TO CATWALK WHERE IT STRUCK EE.
EMPLOYEE WAS A PASSENGER IN THE OUT BY END OF A MANCAR, A PARKING MECHANISM FELL FROM ITS HOLDER TO THE MINE FLOOR CAUSING THE OUTBY END OF CAR TO RAISE ABRUPTLY WHILE DERAILING THE FRONT END OF THE MANCAR. EE'S MOMENTUM AND ITS ABRUPT RAISING AND LOWERING OF THE MANCAR RESULTED IN THE EMPLOYEE'S FALLING ONTO THE EDGE OF THE BACK REST STRIKING THE CENTER OF HIS BACK.

PERSON WAS ADVANCING LONGWALL SHIELDS ON THE FACE DURING PRODUCTION. PERSON WAS LOWERING #8 SUPPORT WHEN A ROCK 23"" BY 16"" BY 3"" FELL FROM BETWEEN CANOPIES AND STRUCK HIM ON TOP OF RIGHT FOOT CAUSING FRACTURE.
USING CHAIN HOIST TO REPAIR BROKEN CONVEYOR BELT, HANDLE OF CHAIN HOIST SLIPPED, STRIKING EE IN THE FACE & HEAD.
WHILE TRAMMING RAM CAR HE RAN OVER A PIECE OF ROCK ON THE MINE FLOOR CAUSING HIM TO BE THROWN UPIN THE OPERATOR'S COMPARTMENT. HE LANDED ON HIS SELF RESCUER CAUSING A CONTUSION TO LOWER BACK.
WHILE STEPPING UP ONTO TRAILER HOUSING WELDING UNIT, TRAILER TIPPED CAUSING WORKER TO TWIST HISFOOT BREAKING HIS LEFT ANKLE AND THE END OF THE TRAILER SMASHED HIS LEFT FOOT BREAKING TWO BONES GOING DOWN HIS FOOT REQUIRING SURGERY AND INSERTION OF A SCREW TO HOLD BONES IN PLACE.
EMPLOYEE WAS PACKING CRIB BLOCKS AND HALF HEADERS 5 BREAKS DUE TO A DOWNED SCOOP. NEAR THE END OF THE SHIFT, HIS BACK STARTED HURTING. WHEN HE AWOKE THE NEXT MORNING, HE WAS HAVING MUSCLE SPAS MS IN HIS LOWER BACK.

EE HAD BEEN PAINTING STEEL BEAMS WITH A BRUSH OUT OF A 5 GALLON BUCKET OF PRIMER PAINT. HE MOVED THE 5 GALLON BUCKET AND WHEN HE PUT IT DOWN, HIS LOWER BACK WENT OUT.
EMPLOYEE WAS IN THE PROCESS OF INSTALLING A NEW 1 1/2"" ROCK DUST HOSE ON THE ROOFBOLTER. HE STATED HIS FOOT GOT TANGLED IN THE HOSE AND AS HE TURNED HE TWISTED HIS ANKLE.
EE WAS ASSIGNED THE TASK OF SALVAGING A SERIES OF VALVES IN AREA II. HE AND ANOTHER HAD CUT NUMEROUS FLANGE BOLTS OFF IN PREPARATION OF REMOVINGA VALVE CLUSTER. HE CLIMBED ON TOP OF VALVE CLUS TER AND CUT OFF SOME MORE BOLTS. THE VALVE CLUSTER FELL APPROX. 7' ALONG WITH THE EMPLOYEE, RESULTING IN A FRACTURE OF HIS LEFT WRIST AND LEG.

HE THOUGHT IT WOULD GET BETTER, SO HE DIDN'T REPORT IT TO HIS SUPERVISOR UNTIL 11-19-01. HE WENT TO MD ON 11-20-01 & MISSED ONE DAY OF WORK.

AFTER WORKING HIS REGULAR SHIFT, THE INJURED EMPLOYEE CLEANED UP AT HOME AND FOLLOWING A SHOWER,NOTICED PAIN IN HIS TORSO. HE SOUGHT MEDICAL ATTENTION THAT EVENING AND THE PHYSICIAN DISCOVERED THAT HE HAD SUFFERED A BROKEN RIB. AT PRESENT, IT IS UNKNOWN HOW THIS INJURY OCCURRED, HOWEVER HE BELIEVES HE MAY HAVE SLIPPED UPON ENTERING AND/OR EXITING THE FRONT END LOADER.
DISCONNECT BELOW FAN WOULDN'T STAY IN. EMPLOYEE PULLED DISCONNECTS BACK SIDE OF SHOP-UP ON HILL.THIS KILLED POWER AT FAN DISCONNECTS. WAS INTENDING TO CHANGE FUSE HOLDER. EVIDENTLY STILL POWER ON THE LINE. GOT INTO POWER AND FELL OFF LADDER.

EMPLOYEE WAS OPERATING A SCOOP DURING LONGWALL TEAR DOWN. HE ATTEMPTED TO LIFT THE HEADGATE DRIVE. THE DRIVE SLIPPED OFF THE BILL OF THE SCOOP CAUSING THE SCOOP TO JOLT. THE EMPLOYEE STRUCK HIS HEAD ON THE SCOOP CANOPY.

THE EMPLOYEE WAS RUNNING A RUBBER AIR HOSE FROM THE PUMP HOUSE TO THE #24 TIRE OF THE #5 KILN, USING THE CONDUIT FOR SUPPORT. WHEN HE ATTEMPTED TO TOSS THE HOSE TO THE PLATFORM AT THE TIRE HIS FOOT SLIPPED ODD THE CONDUIT AND HIS FOOT LANDED ON ROCKS ALONG SIDE THE CONDUIT.
HE WAS SETTING BEHIND BOLTER WHEN A ROCK FELLOUT BETWEEN PINS, 2 1/2' WIDE, 1-1 1/2"" THICK. HITTING HIM ON THE TOP OF HIS HEAD AND RIGHT SHOULDER. HE WAS IN THE MOUTH OF THE #2 ROOM.
OPERATOR STARTED TO DISMOUNT K500 LOADER, LOST GRIP ON HAND RAIL, CAUSING HIM TO SWING TO ONE SIDE HITTING HIS RIBS ON THE TIRE AND HITTING THE GROUND ON HIS LEFT LEG, CAUSING THE LEFT KNEE TO TWIST.

EE HAD JUST FINISHED FUELING A TRUCK-AS HE STARTED TO DRIVE OFF, HE REALIZED THE FOLD DOWN PLATFORM AT THE FUEL TANK WAS STILL DOWN-HE PUT THE TRUCK IN NEUTRAL AND SET THE PARKING BRAKE-AS HE STEPPED OUT ON THE TRUCK LADDER HE HIT THE GEAR SHIFT WITH HIS ELBOW AND THE TRUCK MOVE BACKWARD ABOUT ONE FOOT AND PINNED HIS LEFT FOOT BETWEEN THE LADDER AND THE PLATFORM.

AS HE FELL A WRENCH LANDED WITH SOME FORCE ON HIS RIGHT MIDDLE FINGER. EMPLOYEERECEIVED TREATMENT FOR BILATERAL KNEE SPRAIN AND PAIN IN RIGHT MIDDLE FINGER.
WHILE ALLIGNING & UNLOADING APPARATUS UNDER RAILCAR IN ORDER TO DRAIN THE RAILCAR, A WHEEL BE- CAME STUCK BETWEEN THE RAIL AND THE UNLOADING APPARATUS, WHILE PULLING THE INJURED EMPLOYEE HURT HIS LEFT SHOULDER. DR. SAID HE STRAINED HISLEFT SHOULDER AND TOLD HIM TO TAKE IBUPROHEN AS NEEDED.

AFTER SPRINTING APPROX 300 YARDS TO RETRIEVE AN OXYGEN BOTTLE TO ASSIST A FELLOW EMPLOYEE THAT WAS FEELING SICK AND DIZZY. UPON SEVERAL MINUTES OF OBSERVING THE ILL EMPLOYEE, HE LOST CONSCIOUS NESS. AN AMBULANCE WAS SUMMONED AND HE WAS TAKENTO HOSPITAL.
EMPLOYEE STATED HE WAS WALKING DOWN SLOPE, SLIP ON LOOSE MATERIAL & TWISTED KNEE. EMPLOYEE STARTED MISSING WORK IN NOV.
EE WAS EXITING THE CAT D300D HE MISSED THE SWINGING STEP AND FELL TO THE GROUND.

EMPLOYEE PULLED PINNER INTO PLACE TO START BOLTING, WHEN HE PLACED THE T-BAR THE FIRST TIME HE NOTICED LOOSE ROCK. HE LET THE T-BAR BACK OFF, RELOCATED THE PINNER, THEN PUT THE T-BAR UP AGAINS T LOOSE ROCK CAUSING ROCK FROM RIB TO ROLL OFF AND HIT EMPLOYEES FOOT.
FEED CHAIN ON DRILL TOWER BROKE, SWUNG OVER THE TOP OF THE DRILL AND LANDED ON THE GROUND AND STRIKING EE'S LEFT KNEE AT THE SAME TIME.
EMPLOYEE WAS SHOVELING BELT LINE AND AT AROUND #3 BELTHEAD WHILE SHOVELING EMPLOYEE'S HAND GOT CAUGHT IN BOTTOM ROLLER, BREAKING HIS HAND.
WHILE WORKING AT THE SCREEN PLANT AREA EMPLOYEE WALKED SOUTH TOWARDS THE WAREHOUSES. AT THAT TIME HE STEPPED INTO THE PATH OF AN ONCOMING FRONT END LOADER AND WAS KNOCKED TO THE GROUND WITH TH E BUCKET. HIS LEFT LEG WAS RUN OVER BY THE RIGHT REAR TIRE. THE DRIVER HAD LOOKED TO HIS LEFT & BEHIND HIM TO DIRECT A TRUCK AROUND SO HE COULD LOAD THE TRUCK. LOADER WAS EMPTY & GOING 5 MPH.
EE HEAR A POP IN LEFT SHOULDER. AS HE WAS CLIMBING ONTO TRUCK TO CHECK HIS BED OUT.
EE WAS CROSSING A CONVEYOR BELT, PLACED RIGHT KNEE ON BELT RAIL, FELT PAIN.
EE WAS REMOVING A SHIELDING PROTECTIVE BOX THAT COVERS THE PINION ON THE PEBBLE MILL. AFTER UNBOLTING THE PINION SHROUD, HE & ANOTHER WERE SLIDING THE SHROUD & HE FELT A SHARP PAIN IN HIS BACK
EE WAS WORKING ON ROOF OF MICA MILL WHEN WIND KICKED UP AND BLEW DUST IN HIS RIGHT EYE. FIRST AID WAS DONE AT SITE. THE NEXT MORNING EE STILL EXPERIENCED IRRITATION AND WENT TO THE EMERGENCY R OOM AT THE HOSPITAL.
THE MEN STEPPED OUT OF HIS SHUTTLE CAR INTO A HOLE AND TWISTED HIS KNEE.

HE BRUISED LOWER LEGS BELOW KNEESBUT WALKED TO MANTRIP TO RIDE OUTSIDE. HAD LEGS X-RAYED BUT TEST NEGATIVE - JUST CONTUSIONS.
