Main Portal Elevator doors would not open due to electrical contacts. Problem was repaired and elevator put back into service at 11:24 a.m.

A roof fall was discovered in the return air course of the 2 east 4 right area. The fall is approximately 120' long x 5' high x 18' wide. The fall will not be cleaned up.
A roof fall was discovered int he #4 entry of the 5 west panela; between the #49 and #50 crosscuts, just inby spad 24080. Fall was approx. 70' x 20' x8' long. The area had orignially been bolted with 5' hi-tech bolts; and some 8' super bolts added for supplemental support. MSHA was notified of the fall.
AN UNINTENTIONAL ROOF FALL WAS DIACOVERED BY THE SAFETY COORDINATOR WHILE CHECKING REFLECTOR IN THE RETURN ENTRY OF THE NORTH MAINS @ X-C #11. APPROX. 5 1/2' HIGH, 60' LONG & 17' WIDE. THE AREA WAS DETERIORATING PREVIOUSLY IN THAT SOME FLAKING WAS OCCURRING. THE WALKWAY WAS RE-ROUTED & WHILE AREA WAS BEING RESUPPORTED, THIS ADDITIONAL FALL OCCURRED. AREA IN PROCESS OF REHAB
A roof fall was found in the return aircourse of South Mains ""B"". Fall is approx. 40' long by 18' wide and 8' high. Area was bolted with 8' Install III bolts and resin with bacon straps and wire screen. Air not affected. Location of fall is between 25-A xcut and 26 xcut. Cause of fall was deterioration of beam created by roof bolts due to weathering of the roof.
A roof fall was found at 10 am on 10/30/05. Fall was between entry 2 and 3 at survey station 8+90 7lms unit. Area had been mined sept 12, 2005 and was supported with 6' full resin pins. No inury or property damage resulted. Atea around fall was propped off. There are no plans to clean up this fall. Fall was approx. 9' high x 20' wide x 80' long.
An unintentional fall occurred in #5 entry at #127 xcut. Fall was 20' long, 20' wide, 6' high.
Roof fall occurred on the 3D travelway, entry 5 at the 24+55 intersection. The fall was 70' long, 20' wide and 10' high. No one was injured and passage of men was not affected.
A roof fall was found on 10/30/05 at 7:am at entry #000 survey station 40 + 60 5rms. Fall was approximately 45'long x 45' wide x 25' high. Area was supported with 8' full resin bolts. No injury or property damage resulted but it blocked access to 5r #1 seal. Area was propped off and three new seals will be built around the fall area.
REMOVABLE OVERHEAD PROTECTION; ABOVE THE CAGE & APPROX. 25' TO 30' OF HOIST CABLE WERE DAMAGED ON THE 8'CAGE WHEN THE HOISTMAN PERFORMED A TEST RUN. THE CABLE & CAGE WERE INSPECTED REQUIRING REMOVAL OF APPROX. 40'OF CABLE DUE TO KINKS, NO DAMAGE WAS FOUND ON THE CAGE. AN INSPECTION OF THE SHAFT DETERMINED THAT THE EMPTY SKIP HUNG-UP DUE TO BUILD-UP OF SALT DUST ON THE SHAFT GUIDES.
A roof fall was discovered that is approximately 40' long x 5' high x 18' wide. The fall will not be cleaned up.
Door rod bent on Main Portal Elevator. Door rod was repaired and elevator was put back into service at 6:00 p.m.
MINE EXAMINER FOUND A 40' LONG X 25' WIDE X 5' HIGH UNINTENTIONAL ROOF IN THE #2 ENTRY 20 BREAKS OUTBY THE 001 MMU.
The slope car was inoperable from 6:37 a.m. until 9:40 a.m. due to an electrical malfunction in the control circuit.
Unplanned roof fall above anchorage zone in north main inby survey station 801, 2 crosscuts inby the intersection. Approximately 20' wide, 35' lenght. Fell about 3' above anchorage zone of 4' resin bolts.
A ROOF FALL MEASURING 80' L X 18' W X 10' H OCCURRED 1 BREAK INBY SPAD NO 7179 IN THE NO 4 ENTRY ON 001 SECTION. AREA WAS BOLTED WITH 5' BOLTS.
An unintentional roof fall occurred on the 1st E. sub-main, off 4th N. sub-main on 004 unit #6 entry at spad #13+60. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The fall measured 25' in length by 18' in width and 14' in thickness. The roof fall will be cleaned up and the area will be re-supported.
Oakdale portal elevator was inoperable from the start of the midnight shift until about 4:00 a.m. The problem was a malfunctioning gate latch. Repairs were made by elevator repair contractor. No entrapment at this time. However, at 4""10 a.m. the elevator malfunctioned again and stopped about 100' down the shaft with 18 employees on board. Entrapped for 30 minutes. No injuries.
A ROOF FALL MEASURING APPROX. 40'L X 18'W X 10'H, ONE BREAK INBY SPAD# 7180 IN NO.6 ENTRY ON OO1 SECTION OCCURRED. AREA WAS BOLTED WITH 5'ROOF BOLTS.
A roof fall occurred on the 1B travelway in entry #3 at crosscut #9. The cause of the fall was a hidden slip and weak laminated shale. The fall covered the high voltage cable and the travelway was re-routed around the fall.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 3RD EAST MAIN, ENTRY #6, AT THE 31 + 50 INTERSECTION. THE FALL MEASURED 20' LONG, 20' WIDE AND 7' HIGH. NO ONE WAS INJURED AND THE PASSAGE OF MEN WAS NOT AFFECTED.
10/27/05 at about 6:00 pm occured near the #12 head in entry #4 (track entry). The fall was 18'w x 25'l. x 8'h. the entrance to the fall was breakered off, clean up and rehabilitation has begun.
UNINTENTIONAL ROOF FALL - 6 N SECTION AT #29 3-WAY INTERSECTION. CONVEYOR ENTRY (#3 ENTRY) 20' LONG X 15' WIDE AND APPROX 9' HIGH
SLIP FORM UNDETECTABLE. FALL WAS 20' WIDE X 35' LONG X 40"" THICK. FELL IN THE NO. 6 ENTRY AT SPAD #358.
Hoist went down at 12:40 a.m. Electricians were called in and worked on lower contactor for hoist. Found problems, made repairs to the contactor and returned to service at 9:00 a.m. MSHA was called and told hoist was down at 12:50 a.m. Then MSHA was called and told hoist was back in service at 9:30 a.m. same day.
A roof fall was discovered in return entry of #7 on 10-27-05 at 11:00 am the fall measured app 30' x 20' x 7' high. no personal injuries or machinery was involved. Prelimary investigation revailed that the fall was above the anchoraye point.
An unintentional Roof Fall was discovered on 4 North Mains, #3 Entry Brk. 57. Fall measured approximately 20 ft. wide, 20 ft. long, and 6 ft. high. This fall will not be cleaned up. No equipment was involved. Ventilation was not impaired and there were no injuries.
Sometime before 7AM on 10/26/05 a rock fell from the highwall onto the canopy at belt heading at portal causing the canopy to hit our rail runner and belt. Size is approximately 18 feet wide 6 feet high and 3 1/2 feet deep. NO INJURIES!!
Xcut #9 to #10 had a cutter in the middle of entry. It was bolted with 6' fully grouted resin bolts & 12' cable bolts. The roof was wet. We dangered it of on 10/11/05 & built cribs at the edge of each side. When it fell, it was 62' long x 17' wide x 8' high. No one or no equipment was involved. It will not be cleaned up.
FACE FALL OF #5 ENTRY B-12, 3-RIGHT PANEL OFF OF 1-RIGHT SUBMAIN. DUE TO GEOLOGIC CONDITIONS ASSOCIATED WITH AN ABANDONED CHANNEL DEPOSIT ABOVE THE COAL, THE FALL FELL ON THE FRONT OF THE MINING MACHINE AS IT OPENED UP THE INTERSECTION OF THE #5 ENTRY FROM 4-5 BREAK 12 (B-12) CROSSCUT. THE FALL PROCEEDED OUTBY OVER THE MINER FROM 5-4ENTRIES INTO A 2-WAY INTERSECTION IN #4.
FALL OCCURRED IN THE 4 B ENTRY'S 11 + 05' IN #2 ENTRY. FALL WAS 22' LONG, 20'WIDE & 9' HIGH.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE NORTHEAST MAINS 1 BREAK INBY SPAD STATION # 3762, #7 ENTRY. THE FALL MEASURED 5' H X 30' LONG X 18' WIDE. THIS AREA WAS SUPPORTED WITH 5' TORQUE TENSION BOLTS, OVERSIZE BEARING PLATES (8"" X 8"") AND WAS DEVELOPED 3-31-2005.
A ROOF FALL OCCURRED IN THIS MINE AT APPROX. 5:10 PM IN THE #3 ENTRY (MAINS), ABOUT 10'INBY SPAD #19. THIS FALL TRAPPED THE ROOF BOLT MACHINE BUT NO ONE WAS INJURED. FALL IS APPROX. 90'L X 10-15'W X 6'THICK. PLAN HAS BEEN SUBMITTED TO CLEAN UP THIS FALL.
A roof fall occured in #2 entry of the 2nd nw panel at survey station. 6 + 60. The fall measured 30' long x 19' wide x 10' thick. The areas had been supported with 6' fully grouted resin bolts.
WHILE TRAVELING THE RETURN AIRWAY A ROOF FALL WAS DISCOVERED 7 BRKS OUTBY R5 SEALS, ADJACENT TO E BELT LINE. THE ROOF FALL MEASURED 20 FT WIDE, 40 FT. LONG AND 3 FT HIGH. NO INJURIES WHERE REPORTED AS A RESULT OF THIS ROOF FALL.
A roof fall occurred in the 3A return aircourse in entry #2, crosscut 56 to 57. The cause of the fall was weak laminated shale and was associated with water dripping from the roof. The fall was located in the weekly examiners travel route. The area was timbered and the walkway was re-routed.
While pulling pillars the #1 shield was caught when the top set down. The pillar plan was revised.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THE GALATIA NORTH PORTAL BETWEEN THE 11TH & 12TH CROSSCUT, #2 ENTRY OF THE FLANNIGAN TRAVELWAY. THE FALL WAS APPROX. 25'L X 18'W X 8'THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF WORKERS. THE FALL WILL BE CLEANED UP.
At approximately 7:00P.M., a mine examiner making his weekly return run found a caved area in L-2 entry, 60 feet inby spad #463. (Spad located in L-1 entry) Roof fall started in the intersection of L-2 entry and traveled part way through the crosscut between L-2 and L-3 entry. Caved area is approximately 20 feet wide,45 feet long, and 5 feet high.
A ROOF FALL OCCURRED MEASURING 100' LONG X 20' WIDE X 0-10' HIGH AT SS # 1651, 4000 FEET OUTBY THE WORKING SECTION. NO EQUIPMENT WAS INVOLED, NO VENTILATION WAS DISTURBED, NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.
An unintentional roof fall occurred in the South East Main return, entry #5 x-cut 41 + 60. The cause of the roof fall was a slip and unconsolidated shale above the anchorage point. The area had 6' fully grouted rebar roof bolts and wire matting installed for roof support.
Roof fall on 2nd South in the intersection of #3 entry. 18' wide, 45' high, extending for 20'.
OAKDALE PORTAL ELEVATOR WAS INOPERATIVE FROM 8:02 AM TO 8:50 AM. NO INJURIES, NO ENTRAPMENT. THE DOOR LATCH WAS STUCK & CORRECTED.
ROOF FALL (7000-1-011-05) : LOCATION - 1W#5, Crosscut #97, Entry #6. DIMENSIONS - H:10', W:19', L:40'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointint or slips in roof. ROOF SUPPORT - 5' fully grouted bolts.
A 1' section of the deflection wheel was found on top of the production skip during a routine inspection of the hoist switches. The deflection wheel had deteriorated and a piece broke off the outer rim of one of the spokes. The mine was evacuated for two shifts for the wheel to be replaced. MSHA and State of Ohio notified by phone at approximately 3:00 am.
A ROOF FALL WAS DISCOVERED IN THE 1 SOUTH #5 PANEL, #2 ENTRY, 28 CROSSCUT - INTERSECTION. THE FALL MEASURES APPROXIMATELY 30 FEET LONG, 20 FEET WIDE, AND 10 FEET HIGH. THE AREA IS SUPPORTED WITH 4 FOOT FULLY GROUTED RESIN BOLTS, 10 FOOT POINT ANCHOR BOLTS AND 6 X 6 TIMBERS.
AN UNINTENTIONAL FALL OCCURRED IN #2 ENTRY, 2 X ENTRY AT THE 75 + 10'. FALL WAS 30' LONG BY 25' WIDE BY 7' HIGH.
Elevated levels of carbon monoxide were found coming out of the gob apparently due to spontaneous combustion in the gob. No fire was observed.
Fall app. 20' x 20' x 0 to 10'. No injuries, no blockage of ventilation, no equipment involved. Will not be cleaned up. Breakered off & dangered, plotted on mine map.
Unintentional roof fall 60'(L) x 18'(W) x 7'(T)- one break outby spad # 13379. Found 10/24/2005.
A roof fall occured in the East Mains, crosscut #5, in the pillar line between #5 and #6 entry. The fall measured approximately 30'X19'X6'. The area was supported with 6' tension rebar bolts.
A ROOF FALL OCCURRED IN THE #4 (BELT) ENTRY OF THE 7TH SUBPANEL NORTH AT THE INTERSECTION OF SPAD 29 + 18. THE FALL WAS DISCOVERED AT APPROX 11:00 PM BY THE EXAMINERS AND REPORTED TO THE 3RD SHIFT MINE FOREMAN. THE FALL WAS APPROX 95' L X 7' H X 18' W. NO INJURIES OR DISRUPTIONS TO TRAVEL OF PERSONNEL OR VENTILATION OCCURRED. FALL WAS DISCOVERED ON AN IDLE SHIFT.
During weekly exam of return a fall was encouted in old s. right panel on job side. Fall inby spad #868 40'x 30' 7' intersection.
Truck loaded with coal was coming down the haul road from mine no. 1 to the plant which is a steep incline. Slippery road conditions caused the truck to lose traction. Upon regaining traction, 3 of 6 lugs broke releasing wheel. Driver attempted to go down the hill when the truck flipped over the bank.
Hoist at 1a portal taken out of service for routine schedule maintance.
A roof fall was discovered between the #39 and #40 crosscuts in the #6 entry of A Mains Panel; inby spad #21153. The fall was approximately 80' long; 20' wide and 8' high. The fall was located in the intake escapeway area. The escapeway route was rerouted around the affected area.
The elevator servicing the #6 portal was inoperable due to a malfunction in a safety switch in the bottom of the shaft. The elevator was down from 2:50 p.m. until 4:10 p.m.
Fall found by fire boss. Fall app. 14' x 12' x 6'. App. 7500' outby face, spad #6892. No injuries. No equipment involve. No blockage of ventilation. Will not be cleaned up at this time. Breakered off & dangered. Plotted on mine map. Bolted with 6' torque tension bolts.
ON THE 084-0 MMU, 9A LONGWALL SECTION, AN ALLEGED IGNITION OCCURRED. AN ORANGE-RED COLORED FLAME, ROUND IN SHAPE ON THE HEADGATE END OF THE SHEAR DRUM. FLAME ENVELOPED DRUM WHICH IS 62"" HIGH AND 42"" WIDE. LASTED 3-5 SECONDS. SELF EXTINGUISHING. NO INJURIES TO PERSONS OR DAMAGE TO PROPERTY.
Bolter was bolting the#5 Entry inby Xcut Station 4 + 84. They were on their 4th row in the Face of #5 of a 20' cut. The bolTters went to the back of machine then heard the roof working over the bolter. Then they started to crib around bolter, the roof fell at Face #5 and into Xcut 5-6.
The slope car was inoperable from 1:35 a.m. until 9:10 a.m. since it was off track with a truck hauling the longwall tail drive.
AN UNINTENTIONAL ROOF FALL WAS FOUND IN THE NORTH EAST MAINS, #1 ENTRY, 10' INBY SPAD STATION #3188, BETWEEN FOOTAGE MARKS 28+75 & 26+65. THE FALL MEASURED 150'L X 18'W X 5'H. THIS AREA WAS SUPPORTED WTH 5' TORQUE TENSION BOLTS, ROOF MAT & OVERSIZED BEARING PLATES (8"" X 8""). THIS AREA WAS DEVELOPED 7-29-04.
An unintentional roof fall was found in the Northeast Mains, #1 Entry, 20 ft inby spad station #3181. The fall measured 50'L x 18'W x 5'H. This area was developed 7-23-04 and was supported with 5' torque tension rods, roof mat & oversized bearing plates (8"" x 8"").

An unintentional fall occurred on 3D entries 5+20' #5 entry. Fall was 50' long by 20' wide by 6' high.
A roof fall occured on the 1st west between entries 3 & 4 at survey staiton 23 & 40. The fall measured 50' long x 18' wide x 5' thick. This area have been suppent with 5' fully grouted resin bolts. all appraches to the area have been timbered.
Unintentional roof fall in the 5th east panel occurred at crosscut located at 1380 ft survey station between entries 3 and 4. The fallen material measured 20 x 13 x 6 feet. The support methaod used was 6 ft tension rebar roof bolts.
A roof fall occured in the 1 South, #7 entry, between the pillar line of crosscut #48 and #49. The fall measured approximately 25'X18'X6'. The area was bolted with 6' tension rebar bolts. The area was mined in April of 2004. An undetected slip was determined to be the cause of the fall.
Engine compartment of haul truck caught on fire. Employee attempted to turn off engine and activate fire suppression system. We are unable to determine actual duration of fire, but it is believed to be under 30 minutes. Helena field office was notified of the fire on 10/19/2005.
#2 HEADING ON THE #012 MMU. HAD AN UNINTENTIONAL FALL FROM BREAK #21 TO #22. 10'HIGH X 18'WIDE X 60'LONG. FALL WAS TIMBERED OFF & SECTION WAS RE-LOCATED.
A fall of roof occurred at the 10th East Gateroad, Entry 2, Crosscut 89. The fall measured approximately 30' long x 16' wide x thickness? The fall did not result in any damage to equipment or property, but may have affected longwall bleeder ventilation.
Kuhntown Portal shaft elevator out of service for more than 30 minutes with an employee on it. Out of service due to an electrical problem (transformer) in the head house.
10-19-2005 ON THE #1 SECTION IN THE #5 NEUTRAL ENTRY A ROOF FALL MEASURING 60' X 19' X 5' HIGH OCCURRED AT 10:45 PM. THE ROOF SUPPORT WAS 5' TORQUE TENSION BOLTS & 8' ROPE BOLTS. THE AREA WILL BE CLEANED AND ADDITIONAL SUPPORT ADDED.
Unintentional roof fall in 5th east panel occurred in crosscut 23 between the #8 and #9 entries. The area had been supported with 6' tension rebar bolts. The fallen material measured 30 x 18 x 6 feet.
Unintentional roof fall in the 5th east panel. Fall occurred in crosscut by survey station 1620. It was between entries 6 and 7. The loose material measured 20 x 18 x 6 feet. The method of support was 6' tension rebar roof bolts.
An electrical malfunction occurred int he deskin b-shaft automatic elevator electrical circuit.
The roof fall was 15' wide, 40' long and 6' thick. It was bolted with 4' fully grouted resin bolts. This fall is 1 break inby spad #1242 in #6 entry. This fall has been timbered off and will not be cleaned up. The fall is 4000' from the working section.
Roof fall occured in a 4-way intersection in No. 2 entry at spad 32, 2200 feet from face and 600 feet from portal. Water is seeping thru roof and fall was 70' x 20' x 6 to 8 feet thick.
UNINTENTIONAL ROOF FALL OCCURRED AT THIS TIME BLOCKING 2 MRS UNITS. 20' X 20' APPROXIMENTLY 6' THICK. IN INTERSECTION AND APPEARED TO BE A SLIP ON THE 003 SECTION 5TH LEFT 3 WEST MAINS #2 ENTRY SPAD 1476. DEVELOPED RECOVERY PLAN, SECURED AREA, RECOVERED MRS UNITS AND TIMBERED AREA OFF.
ROOF FALL OCCURRED IN THE 2ND SUBPANEL WEST, #4 TO #7 ENTRY, @ SPAD 60+48. NO INJURIES. FOUND AT 7:30 AM, SECTION WAS IDLE, POWER WAS DE-ENERGIZED DUE TO 1 STOPPING DAMAGED. APPROX. FALL DIMENSIONS: 150'L X 17'W X 6'H. TRAVELWAY WAS NOT BLOCKED, CALLED IN TO MSHA DIST. OFFICE AT 8:53 AM ON 10/18/05. FALL PERIMETER WILL BE SUPPORTED & BRATTICED AROUND.
The #2 hoist failed as the result of a faulty pneumatic timer & proximity switch. Miners evacuated at 8:45 am. The switch & timer were replaced. The hoist was tested & miners returned underground at 10:50 am.
Size of fall was 60' long 20 wide 8' high. Fell in intake at spad #124 2000' from face. Ventilation was affected. Area will be rehabilitated. Passage of miners was not affected. No injuries. MSHA was notified immediately as well as state officials.
An unintentional fall occurred in 3D Entries at 9+00' #4 Entry. Fall was 30' long by 20' wide by 6' high. This area is in recovery stage.
A right to left sequence of pillaring was being used on the 4-LT off a west panel. The first lift was mined from the no. 2 pillar and the mining machine backed out of the lift. The two MRSs were being positioned for the next lift when a rock fell thru the 1st lift and across the entry in front of the MRSs. The fall was 25' long, 4' wide & 8' high & located 50' inby SS 1307.
Unintentional roof fall 1 break inby Spad #13379 in # 5 Entry. Dimensions: 50'(L) x 20'(W) x 6'(T). Roof fall was found @ 3:15PM.
A rock fall occured at the number 6 entry 1 cut inby the number 8 head drive. The fall measured approximately 6 to 9 feet in height, approximately 50 foot long, and 20 foot wide.
Air compressor low on oil at deskins B-shaft.
Fire discovered at 10:15 pm on 62BC11 conveyor belt inside the clinker storage building and extinguished by 11:15 pm.Probable cause: material from the clinker cooler broke apart on 62DP03 feeding 62BC11.Dust from heat exchanger may have covered the hot material and masked the heat from sensors that would have triggered water sprays.Physical damage to belt only--no injury.
Spad 53+28, No. 2 Entry, No. 1 Unit Return, X-cut 77. Approx 70' long, 55' wide 7' high.
Hoistman was running #1 hoist. He brought N. skip down to load, after cager loaded skip he powered hoist up to pull, he realized he had lost power and the weight of the skip pulled S skip into limits before he could stop hoist. Down 12:52 p.m. Running 2:20 p.m.
Fall app. 20' wide, 20' long, 0 to 7' high. No injuries, no equip involved, no blockage of vent. Re-routed the smoke free escapeway around fall in #3 entry - which was also an intake. Fall will not be cleaned up. Breakered off & dangered & plotted on mine map. Was bolted with 6' tor. ten. and 8' & 12' cable bolts.
A rock fall occurred at 13+50 on the #2 unit. This was 1800' outby working section. Fall was from edge #5 entry across panel into old rooms - Return side rooms - the unit was idled until fall was cleaned up in supply road and return.
An ignition occurred at #142 shield, F-11 longwall face. The ignition lasted 2 to 3 seconds and was self-extinguished by the water sprays. There was no damage to equipment and there were no injuries.
AN UNINTENTIONAL ROOF FALL OCCURRED AT SPAD# 8+60 ON THE 3RD NORTHWEST PANEL OFF NORTHWEST SUBMAIN, #2 UNIT (MMU-002-0) #7 ENTRY. THE APPROACHES TO THE FALL HAVE BEEN TIMBERED OFF & THE AREA DANGERED OUT. IT MEASURED 40' X 19.5' X 15'. IT WILL NOT BE CLEANED UP. UNIT WAS MOVED OUTBY FALL AREA.
A roof fall measuring approximately 22' L x 34' W x 13' H occurred at the no. 4 entry, spad no. 7216, section 001. Area was bolted with 5' bolts. Roof fall will be cleaned up.
During a pre-shift examination of the mine a roof fall was found on the # 2 belt system. The roof fall was approximately 20 feet by 18 feet by 6 feet. The fall was cleaned up utilizing a continuous miner and a set of arches were constructed as support.

On 10/14/05 the supervisor was made aware that there had been a ground failure in teh 4-5630 S heading.He and mine engineer went to heading to determine what had happened. Material in the unsupported sill had fallen and coked off at the brow. Unable to determine if it was abouve the anchorage point of the bolts until it could be mucked out to expose the brow.
Slope hoist was inoperable. Wires in control cabinet in engine room shorted out. Repairs were made and hoist was put back in service at 2:15 p.m. on 10/15/05.

An unintentional fall of roof was discovered during the weekly exam on Peters Branch Submains Break #21 at spad #3307. The fall was 140' long, 20' wide and 6' high. The fall extended through the crosscut to #4 and #5 entry.
The continuous miner operator was attempting to make a cut. The coal seam had a rock spar running vertically through the face from 3 to 15 inches thick. A flame occured about mid cut and traveled to the floor. The flame only lasted about 15 seconds.
A roof fall occurred on the 3 Main North in entry number 3 between crosscuts 13 and 14. The cause of the fall was weak laminated shale. This area had previously been cribbed on both ends to protect the adjacent road way and 3MN belt tail.

THE FALL WAS APPROXIMATELY 17' WIDE AND THE HEIGHT AND LENGTH COUNT NOT BE DETERMINED. THE AREA WAS SUPPORTED WITH 8' ROOF BOLTS WITH 2"" WOODEN PLANK AND POSTS. THIS AREA WAS MINED IN JANUARY OF 1972.
A fall occurred at the intersection of #38 break in the #2 heading on 8 haulage extension. The fall was 18 feet by 18 feet and was 4 feet thick in the center.

As the #12 CM Miner received power via the cable, a splice blew out in the long crosscut between #2 and #3 place and ignited gas located in the top of the entry. The cable was hung up at the time in the roof area. No injuries or property damage was incurred.

The hoist at #1 mine had slack in the rope at the drum. The hoist was down for more than 30 minutes while the rope was being tightened at the drum.

A roof fall occurred in entry #1 of the 2nd main north west at survey station 40 + 60. The fall measured 25 ft long x 18 1/2 inch wide x 9 ft thick. This area had been supported with 6' fully grouted resin bolts.
A roof fall took place on the west escape roadway going into 1 Yard North, apprximately 1,000 feet east and 200 feet north of the Service Shaft. The roadway was partically covered. The fall was approximately 50 feet long, 30 feet wide, and three feet thick. No one was in the area when the fall occured. The area has been bolted and meshed and is now safe for travel.

AT THE GALATIA NORTH PORTAL, THE GOVERNOR ROPE ON THE ELEVATOR SLIPPED OFF THE PULLEYS. THE ROPE WAS EXAMINED & NO KINKS OR FRAYED WIRES WERE FOUND. MORE TENSION WAS PLACED ON THE ROPE TO PREVENT SLIPPAGE AGAIN. NO INJURIES WERE INCURRED.
A roof fall was found by the examiner doing pre-shift exam. A four way intersection that had been mined July 14th 1991 and supported with 8' full resin pins failed. Fall was located in entry 3 x-cut 80 main south. No injuries resulted but 100' of belt line was lost. No plans to clean up. Belt line is being rerouted.
AT THE GALATIA NORTH PORTAL, GUIDE ROPES HAD TO BE REPLACED AFTER THEY WERE DAMAGED BY A PIECE OF EQUIPMENT BEING LOWERED IN THE 8TH WEST AIR SHAFT. REPAIRS WERE COMPLETED BY 9:30PM ON 10/1/05. NO INJURIES WERE INCURRED & WORKERS WERE NOTIFIED TO USE THE GALATIA NORTH ELEVATOR AS THE PRIMARY ESCAPE FACILITY.
A correction made by the surveyors resulted in panel width to be 5' wider than required. When measures are taken to open mining to tailgate, excessive width occurs. This process requires implementation of tailgate blockage plan. Condition will correct itself in a short distance as the 5' tapers to zero. Blockage starts as #149.
Two employees were repositioning a conveyor. One was on the loader which was pulling the conveyor. The victim was directing and somehow got caught under the conveyor tire, crushing him.
Employee was looking at sump pump with another employee. He fell backwards through an open hole, falling about 12 ft. into the sump, hitting the back of his head on the way down. He was transported by company ambulance to NNRH ER where he was pronounced dead on arrival.
Employee parked water wagon, got off, didn't chock wheels or set park brake. Vehicle rolled, he either slipped and fell under Rt tire or tire knocked him down and ran up on lower torso.
The worker fell from the bag plant roof where he had been cleaning to the truck dock approximately 17 feet below. The specific explanations for being near the edge or for falling are unknown. Weather conditions and working surface believed to be non-contributory. There were no direct eyewitnesses to the fall. The fall resulted in the death of the worker. Investigation ongoing.
VICTIM WAS ASSISTING WITH INSTALLATION OF NEW BEARINGS ON CRUSHER ROLLS. ONE BEARING HAD BEEN INSTALLED, MAKING PREPARATIONS FOR SECOND. REACHED UNDER THE SHAFT TO REMOVE SOME DEBRIS, WELD ON THE EYE HOLDING THE ROLL BROKE, FELL ON HIS RIGHT HAND. SURGERY PERFORMED ON HIS RIGHT HAND. THE 1ST AND 2ND FINGERS AMPUTATED TO THE KNUCKLE, 3RD AMPUTATED TO THE SECOND JOINT.
While dumping a small dumpster into a larger dumpster with a forklift, the employee's finger was caught between the fork and the fork access on the bottom of the dumpster as he manually pulled the release lever. The employee was transported to the emergency room. The top third of his ring finger was surgically amputated.
Employee alleges that he injured himself while lifting/twisting machine parts. Employee alleges conditions had worsened over a short period of time. The employee stated that there was no specific incident that precipatated the injury.
PRECISE ADJUSTMENT OF OILER REQUIRED POWER DUE TO SYSTEM DESIGN. WHILE MAKING THE FINAL ADJUSTMENT TO THE OILER LINKAGE ROD, THE EE WAS INATTENTIVE TO HAZARDS & PLACED HIS LEFT HAND ON THE OILER SUPPORT BRACE. HIS FINGER WAS IN THE LINE OF FIRE WHEN THE TUBE STRIPPER MOVED. HIS FINGER WAS CAUGHT BETWEEN THE OILER BRACE & THE TUBE STRIPPER.
Employee using a cable come along to realign a tail pulley when he felt a pain in his right groin region.
Claim was reported 2/7/06 - While stemming holes felt a sharp pain in the abdominal area developed a hernia.
The employee has a hernia on the left side of his groin as the result of overextending his left leg to operate the rimpull pedal on a Cat 992G loader.
While operating a shear, the employee lost balance causing his hand to slide under the guard, resulting in the tip of his right fifth finger being crushed by the hold down ram. **Employee received additional treatment resulting in lost time. Employee returned to work 4-18-06 with 13 days lost.**

EE was removing a steel plate from the plow feeder using a manual chain hoist. After a while he tried to move the steel plate with his hands. Suddenly the plate slipped fron the hoist's hook and fell on his left foot.
(LTA) : Main E Unit - Employee was walking toward left transformer. Tripped over a piece of curtain board next to hi-line skid causing employee to fall toward end of the transformer. As employee fell, tried to break fall with left hand. Alledges injury to left shoulder and upper left arm.
INJURED WAS ACROSS SECTION WHEN A PIECE OF ROCK FEL STRIKING INJURED ON BACK AND NECK.
PACKING UP SHAFT WITH BEARING TO MOVE STACK ON A PALLET. LEFT KNEE GAVE OUT CAUSING HIM TO FALL AND THE BEARING FELL ON TOP OF LEFT LEG.
HE WAS CHANGING HOSES ON A BULLDOZER. WHILE GETTING OFF THE BULLDOZER, HE SLIPPED AND FELL TO THE GROUND.
Working on a pebble crusher hydraulic locking post system when he was struck on his arm by the quick disconnect valve fitting. He required surgical flushing and debridment of the site.
Employee was lifting a pad eye for crane rigging and felt a pain in groin area.
Employee was walking when she fell forward and landed on crib block. She was taken to seek medical attention and is off work waiting x-rays results.
2 employees were removing bolts from flanges in circulation leg of K102 crystallizer in order to remove advance pump. When flange wall separated, hot brine water gushed out, burning one employee. Started losing time 11-1-05.
Employee was carrying a core rod and stepped on a hose twisting his knee. Employee had knee surgery on February 16, 2006.
Employee was going thru brattice door, had hand in between door and jam and another employee ahead of him let the door slam on his left hand. Employee did not start losing time until 11/9/2005 due to results of MRI - torn ligaments.
Employee: Cowling Bottom Tool Room - Employee was getting tool box and come-a-long out of belt crew tool box. The lid of the box (30"" x 36"") fell and closed on right hand. No fractures. Contusion/bruise type injuries to right hand and right index finger.
Alledged incident was to occur while this employee was walking along a parked scoop. He tripped and fell against the bucket causing an contusion to his ribcage. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, miner proficiency a factor, safety equipment not a factor, mining equip/systems not a factor.
EMPLOYEE WAS ADDING STEEL TO DRILL A HOLE WHEN THE BOOM EXTENDED & THE JACK LUNGED FORWARD & CAUGHT EE'S HAND BETWEEN THE STEEL & THE CANOPY.
MEMBER CAUGHT BETWEEN 2ND SET OF AIR LOCK DOORS AND RIB.
THE CREW WAS CLEANING UP A ROOF FALL, A 4"" THICK SLAB OF ROCK 4' BY 3' SLID OFF OF THE ROOF FALL AND HIT EMPLOYEE IN THE BACK OF HIS RIGHT LEG SEVERAL INCHES ABOVE HIS ANKLE. THE ROCK BROKE HIS ANKLE AND KNOCKED HIM DOWN.
EMPLOYEE STEPPED OUT OF MANTRIP AND TWISTED ANKLE.
Dismounting crane, stepped into ground depression, twisted ankle. Did not seek medical attention until 10/31/05. Returned to work, no lost time & no restrictions. Re-check on 11/2/05 for continued pain. Time lost starting 11/4/05.

Rocks from the side of a highwall fell striking the back of an excavator breaking the front and rear windows of the cab. One piece of rock pushed the operator seat forward requiring his seat belt to be cut off. The operator was struck by a piece of the rock in the back of his head requiring stitches.
He was unloading a hydraulic cylinder and strained his left shoulder. He started missing work on 11-1-05.
EMPLOYEE SAID THAT LOADER OPERATOR DROPPED ROCK TOO HARD IN TRUCK BED.
After employee was finished putting up pin, his partner told him that a rock was on the canopy. As employee lowered the conopy the rock slid off, striking him in the lower back and hip area. The rock was approximately 1' x 2' x 1/2"" in size.
EMPLOYEE WAS LOADING FLYASH WITH A SMALL AMOUNT OF LIME INTO A FRONT END LOADER BUCKET. THE MATERIAL CAME OUT OF THE STORAGE TANK ALL AT ONE TIME CAUSING THE ASH TO SPLASH OUT OF THE LOADER BUCKET. SOME OF THE MATERIAL GOT INTO EE'S RIGHT EYE.
AN EMPLOYEE WAS LIFTING A TIMBER 5 X 7, SLIPPED ON A ROCK TWISTING LEFT KNEE. *EMPLOYEE'S LAST DAY AT WORK WAS 10/31/05.
When the bag fell off the Bagger it had too much air in it and hydrate lime spewed out of the bag when it hit the conveyor. Lime went under the employee's safety glasses into his left eye. Employee went to the E.R. on Saturday afternoon.
base of forklift struck a non-operable overhead crane which fell and landed on ee's legs. Inj employee's stomach/chest area ended up pinned against welding table.
Employee stated that he was attempting to remove a rock from between the tailgate and the truck bed. When the rock dislodged, the tailgate swung back while he was holding onto pin and struck the latch.
Employee stopped to help the miner operator pull miner cable out of the roadway and strained his arm.
Employee was stacking pallets with 100 lb bags of bentonite. Bags were traveling down conveyor to throwing plate, EE was unable to remove quickly. He pressed emergency stop, at which point bags continued to slide down throw plate. He reached to grab bag when it made contact with wrist and throwing plate.
Employee was driving down the main north travelroad, when a rock fell from the roof and struck the victim on the forehead, between the eyes, causing a laceration and stitches.
WHILE ROOF BOLTER WAS BEING TRAMMED INTO FACE A PIECE OF ROCK FELL STRIKING INJURED IN LEFT SHOULDER.
EMPLOYEE WAS MAKING A POWER MOVE ON WEST SECTION AND WAS PULLING SLACK FROM THE FEEDER CABLE. HE ENCOUNTERED A LITTLE PAIN IN HIS RIGHT HIP.
EMPLOYEE REPORTED: RECOVERING KENNEDY METAL STOPPING, WHEN IT TELESCOPED TOGETHER, A SHARP EDGE CONTACTED HIS FINGER RESULTING IN A LACERATION REQUIRING 5 SUTURES.
Employee was lifting cable and suffered hernia strain.
He was installing brakes on water truck. When lifting brake drum, his hip popped out of place.
INSTALLING AN ACCESS LADDER TO CATWALK ON DOZER TRAP CONVEYOR. STEPPED OFF DECK ONTO A ROCK. ROCK MOVED AND EMPLOYEE LOST BALANCE AND HURT LEFT KNEE.
EE was stepping around other employee when he lost his balance, misplaced his left foot and fell to the floor. In falling to the floor, EE grabbed the other employee, bringing him to floor with him. Misplaced foot was broken.
Employee was climbing off a conveyor frame, mis-stepped onto an electrical cord, and twisted knee.
He was operating a lo-trac diesel loader while removing a belt drive head roller. He bumped the lo-trac into the rib & a piece of the rib rolled off & struck him in the back & hip. He has since complained of pain in his back & hip. on 11-30-05, he was diagnosed with a compression fracture at T6 & severe arthritis of the hip. He began missing work on 12-9-05.
The employee went to put a block on the brattice he was working on and felt pain in his left shoulder and the block fell and struck him on his left shoulder.
EMPLOYEE STATED HE WAS SWINGING A SLEDGE HAMMER ON #1 ORE BIN. HE FELT PAIN IN LOWER BACK.
Employee called supervisor on radio and said he needed to go to the hospital and have his neck checked. The injured employee told his supervisor that the loader operator had slammed his truck with a big rock, causing injury to his neck.
The injury occurred when the employee was riding underground seated at the rear of the mancarrier. As the mancarrier was travelling it hit a bump on the road causing the employee to bounce up off the seat and strike his head (hard hat) against the roof of the mancarrier.
Employee was attempting to close metal sliding railcar load door under palletizer incline. Employee was behind door edge pushing. Door stuck due to dirty rollers. When employee leaned in on door to push harder, door broke loose suddenly causing his feet to slip out behind him. Employee fell onto concrete floor landing on left side. X-Ray diagnosed with broken left hip bone.
EMPLOYEE WAS CLEANING CORDS AND A PIECE OF DOG CHAIN THAT WAS WRAPPED AROUND THE TAILROLLER SHAFT AT THE 4I TAILPIECE. THE BELT WAS TURNED OFF. WHILE ATTEMPTING TO REMOVE THE OBSTRUCTION, THE BELT UNEXPECTEDLY DRIFTED CAUSING HIS HAND TO BE CAUGHT. HIS REACTION WAS TO PULL HIS HAND OUT. THIS CAUSED THE LACERATION.
Oper. moving material with loader and bottomed out seat, employee didn't seek medical attention at that time but did seek it 110905 was issued a return to work from the physician. He saw a Dr. on 111005 and again was issued a return to work with no restrictions. The he saw a Dr. on 112805 and was diagnosed with a bulging disk and was not allowed to return to work as of 112805.
The miner was mining in 3 right pillar, made 2nd run and backed out to timber off when employee started yelling, co-workers ran back to him they found the mbc track was on top of employee's left foot. Accident resulted in fractures.
WHILE WALKING BESIDE THE 008 SECTION BELT, EE FELL INTO BELT CAUSING HIS RT HAND AND SHOULDER TO BECOME INJURED. HE HAS CONTINUED TO WORK. HIS FIRST DAY MISSED WILL BE 12/19/05.
EMPLOYEE WAS IN AN AREA OF THE MACHINE WHERE HE WAS NOT SUPPOSE TO BE AT, THE OPERATOR DID NOT SEE HIM. AS THE CARRIAGE WAS MOVING FORWARD TO GET ANOTHER FLIGHT, THE EE POSITIONED HIMSELF BETWEEN A STATIONARY LIFT JACK & THE MOVING CARRIAGE WHICH PINCHED HIM & FRACTURED HIS PELVIS. THE OPERATOR HEARD HIM YELL & STOPPED THE CARRIAGE.
Injured and co-worker were working on a 785 rock truck engine. They had to work underneath the engine and were having problems trying to remove a piston. During the process, he overexerted or twisted his back while pushing up on a connecting rod. The pain grew worse as the shift proceeded. He worked the next shift but had to seek medical attention on Monday, 10/31/2005.
EMPLOYEE WAS CLEANING WITH THE VACUUM SYSTEM AND LIFTING LARGE HOSE CAUSED PAIN IN SIDE.
While splitting stone missed chiesel and hit left ankle with 4lb hammer.
Severe jarring to lower back while pushing rock with motor grader.
The mechanic was performing maintenance on a wheel loader. He was walking in front of the loader with his hands full of tools and slipped. He hit his elbow on the way down to the ground. He was o.k. to work the rest of the day and weeks later. It slowly started to swell and he sought medical attention on Oct. 26. The doctor said it was a bruise that possibly needed drainage.
Was unhooking trailer when hitch slipped forward and smashed foot between trailer and bumper.
While drilling, felt pain in stomach and groin areas. Lost time as of 2/9/06.
EMPLOYEE WAS USING A CASE STEER (BOBCAT). THE EE EXITED THE CAB OF THE CASE WHEN THE LANYARD FROM THE HARNESS HE WAS WEARING WRAPPED AROUND THE LT. CONTROL HANDLE CAUSING THE CASE TO ENGAGE THE LT. WHEELS, SPINNING THE EQUIPMENT AROUND CLOCKWISE. THE EE'S RT. ANKLE WAS DRAGGED UNDER THEN TRAPPED UNDER THE LT. SPINNING FRONT WHEEL. A CO-WORKER FREED THE LANYARD.
Employee was performing arc welding near a highly reflective surface. Welding flash was being reflected off this surface through the back of his welding helmet/shield & was able to strike both of his eyes causing a welding arc flashburn. Employee sought medical attn. & was instructed to avoid work for four days.
MACHINE BACKFIRING AFTER NOT BEING STARTED FOR A FEW DAYS, COLD WEATHER
Employee was working on the secondary discharge screen when the counterweight slid off the steps and struck him on the right ankle causing swelling and bruising. The 2 week followup exam reveiled a fracture of the heel.
Employee was loading a scoop with belt structure when he felt pain in his back. He continued to work thru 11/8/05.
Employee was attempting to put a crib under a feeder which was being blocked up on section tail piece when feeder tilted to left away from him. He was pushing a crib block under the frame when feeder struck crib block pinching his right thumb between feeder and crib block, crushing thumb.
RUNNING CABLE TOOLS, CAME OUT OF HOLE WITH TOOLS, MOVING TOOLS TO SIDE, RIG DID NOT LET TOOLS MOVE OVER. TOOLS CAME LOOSE ALL AT ONCE AND CAUGHT FINGER BETWEEN BOARD & TOOLS.
EE had allowed mud to accumulate on his work boots. He was walking on a concrete pad and slipped as result of the mud accumulation. He felt a pain in his left groin area. His personal physician diagnosed him with an inguinal hernia. Surgery to repair will be done on November 10, 2005.
EMPLOYEE STATED THAT HE WAS INSTALLING NEW PVC CONDUIT TO EBR-48 DRYERS AND PVC CLEANER SPILLED INTO THE EMPLOYEES GLOVES. HE CONTINUED TO WEAR THE GLOVES UNTIL FINISHING THIS TASK. WITHIN 3 DAYS HE EXPERINCED A SEVERE RASH ON BOTH HANDS. THE CLEANER MATERIAL HAD IRRITATED HIS SKIN CONDITION. HE SUFFERS FROM PSORIASIS.
Employee stepped down of a Komatsu 400 Excavator and rolled his ankle.
EMPLOYEE WAS OPENING THE SLIDE GATE ON A RAIL CAR. THE BAR HE WAS USING FELL ONTO A PILE OF CEMENT AND SPLASHED UNDER HIS SAFETY GLASSES. SENT TO SEE A DOCTOR AND WAS DIAGNOSED WITH CORNEAL ABRASION. SAW A SPECIALIST ON 10-27 AND PLACED ON NO DUTY. BACK TO WORK ON 10-31-05.
Employee was using a loader and was pushing brush up into a pile, the loader was an open cab. While doing this the employee was struck by a stick that had come up through the cab and struck him in the head, rendering him unconscious for approximately one to two minutes
Testing electrical panel. Employee's hand received an electrical flash burn.
Employee passed out and fell approximately 8 feet off of the top platform off an off-road dump truck (onto sand ground). Employee injured head and bruised ribs. Employee was transported to local emergency room where he was tested over the legal alcohol limt.
Employee was shoveling around a crusher feed belt, which was feed by another belt. He noticed some material had built up in the rock box and was spilling over onto the ground. As he attempted to clear the material with a shovel, he placed his hand on the rock box and was struck on the left hand index finger tip by a rock.
EMPLOYEES WERE GROUTING 30' CABLE BOLTS, PUMPING GROUT TO THE TOP OF THE HOLES USING 7/8 X 3/4 POLY TUBE AND GROUT PUMP. SECTION OF TUBE OUTSIDE OF BOLT HOLE FAILED (RUPTURED) CAUSING GROUT TO SPRAY INTO INJURED EMPLOYEE'S EYES. HE WAS WEARING SAFETY GLASSES AT THE TIME.
Employee was building stoppings on 11 section when block fell striking thumb on right hand.
While emp was descending down the stairs from the catwalk, stepped on a rock and injured left knee.

THE INJURED WAS SPREADING ROCKDUST BY HAND. HE WAS CARRYING 1/2 LB. OF ROCKDUST DOWN A SLICK GRADE WHEN HE SLIPPED AND FELL. THIS RESULTED IN A FRACTURED RIGHT ANKLE.
Employee tripped while carrying belt roller. Roller fell on lower leg and ankle.
Employee was operating semi-automatic palletizer when he started to feel pain in his right armpit area. Pain was tolerable; finished his shift and denied medical treatment until the following day.
Employee jumped 3 feet off the track of a Lima 2400B drag line to the ground and sprained his left ankle.
EMPLOYEE WAS RIDING IN A BROOKVILLE MANTRIP THAT SUDDENLY LOST ITS BRAKES CAUSING IT TO ROLL BACKWARDS 1350' STRIKING ANOTHER MANTRIP.
EMPLOYEE WAS STANDING IN A CROSS-CUT. HE MOVED OVER NEXT TO THE RIB TO LET A SCOOP BY WHEN A PIECE OF ROCK FELL OUT OF THE CORNER OF THE RIB HITTING HIM ON THE RIGHT LOWER LEG FRACTURING THE SMALL BONE.
Employee was struck on the top of his head with a piece of draw slate that fell from between the roof bolts. The piece of rock measured 2 feet by 3 feet by 2 inches. This resulted in a strain to his neck.
EMPLOYEE WAS A PASSENGER IN A BROOKVILLE MANTRIP. WHEN THE OPERATOR ATTEMPTED TO STOP HE SUDDENLY REALIZED HE HAD NO BRAKES CAUSING THE MANTRIP TO ROLL BACKWARDS 1350' STRIKING ANOTHER MANTRIP AT THE END OF THE TRACK.
Driller was lifting a DTH hammer in the winch line. As the hammer came through the table it caught on the table which caused it to start swinging. Hammer was swinging towards the window on the drill cab so driller reached out and grabbed it to stop it from hitting the window. When he did this the weight and momentum of the hammer pulled something in his mid-back.
Foreign object entered eye while chipping weld slag or grinding weld.
EE was climbing through a door in the intake stopping in Unit #1. He was plastering the stopping and he was stepping through the door to get to the other side of the stopping to plaster the back side. During this process he stepped on a wooden roof board laying on the mine floor near the stopping door. He twisted his right knee. He fininshed the shift and saw a doctor the next day.
EE stated they were changing screens and he pulled on the bottom screen when at that time he felt a sharp pain go through his left shoulder

EMPLOYEE WAS ON BAGGING CREW STACKING 50# BAGS OF PRODUCT WHEN ONE BAG SLIPPED OFF PALLET & HE TRIED TO CATCH IT WITH HIS LEFT HAND BEFORE IT HIT THE FLOOR. EE DIDN'T EXPERIENCE ANY DISCOMFORT UNTIL LATER IN HIS SHIFT & SOUGHT MEDICAL TREATMENT THE FOLLOWING DAY, 10/25/05.
INJURED WAS CLEANING THE CANOPY TOP OF LONGWALL SHIELD. A ROCK FELL WHILE INJURED WAS PULLING ROCK FROM THE CANOPY. THE ROCK STRUCK INJURED ON THE RIGHT HAND RESULTING IN A FRACTURE AND LACERATION.
Employee was grinding - using grinder - & got a piece of metal in his right eye.
THE EMPLOYEE WAS ATTEMPTING TO CLEAN OUT A CHUTE ABOVE THE HAMMER MILL. HE CLIMBED UP ON THE CONVEYOR BELT THAT WAS WET & HAD SOME MATERIAL ON IT, HE THEN LOST HIS FOOTING & FELL APPROX. 8'.
EMPLOYEE WAS IN THE TIGHT CONFINES OF A CLUTCH COMPARTMENT ON A DRAG LINE TRYING TO BREAK A BOLT LOOSE. PULLED SOMETHING IN LOWER RIGHT GROIN. WE ARE NOT SURE IF PROBLEM CAME ON THE JOB OR NOT. WE HELD UP FILING UNTIL WE KNEW THAT THE EMPLOYEE WAS BETTER AND HAD NO MORE PROBLEMS.
Employee was raising the hood on truck #107. He lost his footing and fell backward off of the fender and landed on his right hand breaking his hand or finger.
He was going to work on conveyor chain on the miner and rib rolled out on his right foot.
EMPLOYEE WAS HELPING WORK ON HAULER, HOOD HAD BEEN REMOVED LEANING UP AGAINST HAND RAIL. THE WIND BLEW THE HOOD OVER ON EMPLOYEE STRIKING HIM ON THE LEFT SIDE OF HIS HEAD.

Injured was operating the roof drill when the drill steel dropped out of the drilled hole striking him on thumb and index finger.
THE EMPLOYEE WAS LEANING A 55 GALLON DRUM OF OIL OVER ON CONCRETE WHEN IT SHIFTED AND SLID BACK ON THE CONCRETE CAUSING HIS SHOULDER TO BE PULLED.
Employee was rolling up rubber hose that had previously contained caustic soda. Some of the caustic soda residue from within the hose contacted his pants. The individual washed off his pant leg however some of the chemical contacted his skin causing chemical burns and irritation to his lower leg.
Cleaning the cone crusher with a pry bar and his finger was squeezed between the bar and guard on the cone crusher. He lost his nail and received 17 stitches.
Replacing pump and water line in #95 c/c area in approximately 3 feet of water when right foot slipped between two water lines causing me to fall backwards and injurying right ankle, right knee and hit left shoulder and neck into belt structure.
The retail tractor trailer's load of ash stuck in the trailer as it was dumpping its load and cause a unbalance to the trailer which rolled onto its side. The truck was over loaded by 6440 pounds.

While cleaning out gate of #9 screen and reversing the gate flow, gate swung forcing bar forward pinching the employee's finger between the bar and I-Beam injuring the tip of right little finger.
Employee was operating a Caterpillar 980 loader loading rip rap when a rock fell off the bucket. He did not notice the rock and backed over it, injuring his back and left shoulder.
Steps into crusher control van to clock out for the day, hung shoe string.
EE was performing mechanical duties a Kiln's # 6 cooler. After he finished the task his proceeded to keep two manual chain hoists to the tool room. while he was passing by underneath a conveyor he began to feel back pain. ***The employee reported the accident on 11/22/05
EE WAS OPERATING THE EAST CRANE COMING BACK WITH STARTER SHEETS DURING THE HARVEST. HE SLIPPED ON THE GRATING WHICH WAS WET AND INSTALLED UPSIDE DOWN. THE EE FELL BACKWARDS AND STRUCK HIS HEAD ON THE BUSS BAR WHICH CONNECTS THE TANKHOUSES. HE WAS DIAGNOSED WITH A BRUISE TO THE HEAD AND SHOULDER AND TAKEN OFF WORK. ONGOING
As Employee was grinding welds off a roller frame, a metal fragment was dislodged and thrown back at his face, where it entered his eye.
After servicing the 2105 haul truck, employee was going back up the ladder when the step broke. He pulled his left shoulder trying to hang onto the ladder.
WAS WORKING ON A PIECE OF EQUIPMENT WHEN A PIECE OF THE EQUIPMENT CAME OFF CAUSING INJURY TO THE 5TH DIGIT ON THE LEFT HAND.
Standing on track of machine and slipped off and fell on left shoulder.
Employee was performing cleaning duties with a chipping hammer at Kiln area. After a while a hard piece of cement accumulated from a high part of the structure fell down due to vibration of the hammer. The cement struck employee in his right cheek.
A mechanic was attempting to access the hinge area of a scaling unit to fix hydraulic oil leak. He exited the cab and chose a path that lead him over the exhaust system, fender and other components. Upon his final step to hinge area, which was soaked with oil, his right foot slipped from under him and he fell approx. 4' to the ground breaking the upper portion of his right arm.
Employee was getting out of a Shuttle Car and twisted his right knee. This did not become a Lost Time Accident until 11/30/2005.
Employee was getting out of a battery jeep when his foot slipped carrying him to fall, hitting his left knee against the deck of the scoop. He didn't go to the dr until 2-13-06 when mri showed a tear. He had surgery and has been off work since 2-13-06.
EE REPORTED: OPERATING A ROOF BOLTER MACHINE, BOLTING FACE AREA. WHEN REMOVING A 6' RESIN BOLT FROM THE MACHINE'S STORAGE TRAY, THE BOLT HUNG IN THE TRAY. HE JERKED TO FREE THE BOLT, THE BOLT CAME OUT SUDDENLY CATCHING HIS FINGER BETWEEN THE END OF THE BOLT AND THE FRAME OF THE MACHINE'S ATRS RESULTING IN A SMALL LACERATION REQUIRING 3 SUTURES.
Shoveling belts and belt grabbed shovel, pulling hand into roller.
While installing a water hose to the section water manifold, the manifold broke free from the water line and struck individual in the left shoulder and side of face. This chipped his bottom left wisdom tooth and caused a hair line fracture to jaw.
Quality control employee emptying sieve screens and a small particle of rock or dust bounced into his eye under his safety glasses.
THE INJURED EMPLOYEE WAS CUTTING OPEN A BAG OF ROCKDUST WHEN HIS KNIFE SLIPPED RESULTING IN A LACERATION TO HIS RIGHT THUMB. THE LACERATION REQUIRED 7 SUTURES.
The pinner was hung up in the bottom of a swag. The scoop backed down the hill to pull the pinner out. As the employee was hooking up the obain the scoop rolled back catching his leg between the bumper of the scoop and the atrs on the pinner causing a fracture.
Employee was checking for a chipped tooth on the bucket on the shovel. He pushed a rock out of the way which in turn fell on his left hand & fingers.
EMPLOYEE WAS WORKING BACK TO BACK WITH A FELLOW EMPLOYEE PUTTING A CRIB TIE IN AN ARCH WHEN THE CRIB TIE FELL OVER THE FELLOW EMPLOYEE'S SHOULDER AND STRUCK EMPLOYEE IN THE CALF.
CONDITIONS WERE WET. EE WAS CLIMBING LADDER ON MACHINE USING 3 POINTS OF CONTACT. HIS FOOT SLIPPED OFF LADDER & HE FELL OFF LOADER. EE TRIED TO HOLD HIMSELF WITH BOTH HANDS BUT WAS UNABLE TO. HE FELL TO GROUND CAUSING BACK PAIN.
Employee was throwing roof bolts up to the front of the roof bolter when he felt a sharp pain in his right shoulder. He began missing work due to this injury on 11/17/05. RTW 11/18/05 on light duty.
EMPLOYEE WAS HANDLING MINER CABLE WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
Became reportable on 02-02-06 when he was removed from work by physician prior to surgery. Employee was getting off of continuous miner, slipped (didn't fall), injured his shoulder when he caught himself.
EE was hanging the cable of a joy 21 shuttle car and came into contact with electricity at a splice.
STEPPED DOWN FROM THE DOCK OF A SMALL FUEL TRAILER. STEPPED ONTO THE TONGUE OF THE TRAILER. THERE WAS DEW ON THE TONGUE OF THE TRAILER, HIS FOOT SLIPPED & HE FELL ACROSS THE TONGUE.
Employee strained lower back while hanging miner cable.
Employee complained that he had something in his eye after he had left work. He had been grinding metal earlier in his shift and may have gotten a metal particle in his eye when he wiped his face with his sleeve.
While checking placement of inner part of brake assembly it came down onto his right hand.
Employee was climbing up on a Caterpillar 785 Rock Truck to perform some mechanical duties and as he pulled himself up on the ladder he felt a pain in the lower part of his back. He is scheduled to take physical therapy for a strained muscle in the lower back and is not released to come back to work yet.
Employee was dismounting a 777D when his foot slipped off the step. Employee caught himself, hurting his back and stomach.
Employee was sorting out structural steel moving around pieces when a beam measuring about 24""x10""x24"" fell over on his left foot. He was wearing metatarsal boots, but a piece of the steel caught his foot below the metatarsar ont he side near the ankle.
Employee stuck his left arm through the window of a feed chute. Material broke free pinning his arm inside the chute. He was pinned 2 hours and 45 minutes and immediately transported to the hospital upon extraction. Waiting word on his condition.
5 NORTH FACE - #4 ENTRY. WHILE CENTER BOLTING, MATERIAL FELL FROM TOP (FROM BETWEEN ROOF STRAPS) AND STRUCK OPERATOR ON RIGHT SHOULDER.
Employee applied diesel fluid into the wood burning stove. He lit the stove with his lighter. The starter fluid vapors ignited, causing flash burns on his left and right hands and forearms. He sustained first and second degree burns which covered about 5% of his body.
WHILE PERFORMING MAINTENANCE ON THE HIGHWALL DRILL, THE INJURED SLIPPED AND GRABBED THE EXHAUST PIPE TO CATCH HIMSELF AND PREVENT A FALL. BY GRABBING THE PIPE HE BURNED HIS ARM AND HAND.
While employees were getting instructions at shack, a rock app 3"" - 12"" thick and 2' x 3' long, fell striking 2 employees on their heads shoulders and backs.
While preparing to install the rib pen of his fifth row of bolts a rock measuring 10 1/2' long 30""wide & 6"" thick fell along the rib line parallel to the entry striking him on the left ankle and lower leg causing a sprain/strain to the ankle.
Employee was clearing rocks from the grating on a drive over feed hopper. Employee felt soreness in his left shoulder, elbow and hand the next day. Employee has not returned to work due to pain in shoulder.
While employee was working on the stockpile he dismounted his dozer and climbed up the side of the berm to check his over dump. As he was doing this he slipped and fell hitting his left knee on a rock. Employee has taken time off work and is currently ongoing.
The employee was struck by a rock measuring 10 1/2'long 30"" wide &6"" thick, from between the rib pen and rib. He recieved the blow to his left head and shoulder knocking him to the ground and causing fractures to both ankles.

After placing ram car battery on charge, when lifting the battery lid for ventilation purposes he felt pain to his right shoulder. Pain and discomfort continued. Sought medical attention on 11/21/05. When under doctor's care he started missing scheduled work days as of 11/23. Initial diagnosis is a possible torn rotor cuff.
Miner was in the 6500-670 underground Normet supply shop area getting supplies. He was loading a 50' compressed air hose onto his tractor and sprained his right ankle.
Employee stated that strained his back while lifing a belt roller.
The injured employee was crawling toward the roof bolt machine when his left foot slipped off the coal and became caught underneath the wheel of the machine.
While repairing and cleaning inside of 9A Ramsey Feeder employee stopped the feeder to examine it, thought the feeder had stopped turning then put his right hand into the paddle wheel of feeder, but it was still coasting to a stop which resulted in severe cuts to all fingers on employee's right hand.
Emp was cutting pipe; as coworkers moved cut pipe, it caused a piece of pipe tomove towards the emp, pinching his left leg between two pieces of pipe. Emp was diagnosed with fractured left fibula, placed in a boot and on crutches and told to remain off work. Emp also suffered a burn to the left calf that was grafted.
EMPLOYEE WAS EXPERIENCING SORENESS IN HIS LEFT WRIST AT THE START OF SHIFT. WHILE PULLING BARS, THE PAIN INCREASED AND TRAVELED UP HIS ARM. EMPLOYEE WAS TAKEN TO THE DOCTOR'S OFFICE AND AN MRI IS RECOMMENDED. CURRENTLY ONGOING.
Injured employee was walking by an area where other employees were doing a belt splice. A metal fragment flew out from the ginding and entered the injured employee's eye.
Employee was lifting a 4"" pump discharge hose. He pulled muscles in left shoulder. (Surgery required - 1/17/06)
THE INJURED WORKER SLIPPED ON AN OILY SURFACE WHERE HE WAS STANDING TO REPLACE THE LEAKING SEAL AND TWISTED HIS LEFT KNEE. HE DID NOT REPORT THE INJURY FOR A FEW DAYS AS HE THOUGHT IT WAS A MINOR INJURY. AFTER DR VISIT & AND MRI IT WAS DETERMINED THAT SURGERY WOULD BE REQUIRED TO HIS KNEE. THIS WAS DETERMINED ON 11/3/05.

Switching an air valve to the closed position, he had to remove a rock from the valve before switching the air line. As soon as the rock was removed the valve closed on his hand.

A ROCK MEASURING 12' X 3.5' X O"" TO 7"" THICK FELL AND BROKE OVER THE TRAM CANOPY OF THE MACHINE. ONE OF THE PIECES STRUCK EE ON THE SHIN CAUSING A LACERATION AND PARTIAL AVULSION THAT REQUIRED SUTURES TO CLOSE.
HE WAS CHANGING BATTERIES ON GOLF CART, WHEN HE STRAINED HIS SHOULDER WHILE LIFTING BATTERY OUT OF THE GOLF CART. **STARTED MISSING WORK ON 01-16-06 **

EE stated that he was roof bolting when a piece of draw rock fell and struck him on the right hand. He was xrayed and showed no fracture.

Employee was shoveling coal off of a mining beam that was on the fork lift when the beam caught his right foot mashing his toes.
Assisting shift repairman clear choke #1 separator. Descended stairs to lockout equipment. Going back up steps to job site; felt sharp pain in right calf.
Employee threw concrete form, form caught his vest.
The worker went to a mill bin to get the cement to flow. He attached high pressure to the internal air pads and tapped on the bottom to loosen the material. While checking for flow at the discharge, the seal came out from under the holding seal band and blew cement on him knocking off his safety glasses and getting into his eyes.
Injured was lacing hi line when he stepped on a muddy piece of belt and fell on right arm causing a sprain on right shoulder.
Employee was coupling an air hose to adjoin two railcars. When finished, the employee rose from the squatting position and felt a pop in his lower back along with pain. Doctor's report showed that the employee had chronic back pain and spasms. The employee had just returned to work on 10/17/05 from being off 3 1/2 months due to a non-work related back injury.
While employee was re-positioning roof bolter to start the next inby row of roof bolts he trammed roof bolter over top of left foot.
CREW WAS LEAVING SECTION TO GO OUTSIDE, A PIECE OF 4 INCH WATER LINE WAS LYING ALONG SIDE OD TRACK. IT CAUGHT ON MANTRIP AND CAME INTO DECK STRIKING HIM ON THE RIGHT ARM AND SHOULDER. HE WORKED UNTIL 10-18-2005 AND CALL IN STATING THAT HE WAS GOING TO SEE A DOCTOR. AT THIS TIME HE WAS TAKEN OFF WORK.
EMPLOYEE WAS WALKING ON CONVEYOR BELT. HE LOST HIS FOOTING. EMPLOYEE FELL 15 FEET 5 INCHES TO THE GROUND. POSSIBLE BACK, LUNG AND RIB INJURIES ON LEFT SIDE.
EE stated that he was putting oil in the shuttle car when the hadle broke on the can of hydraulic oil. The can struck his left hand and cut it requiring 5 stitches.
Employee stated that he strained his back while pulling on belt structure.
Employee knelt down on catwalk to remove debris. Knee popped causing severe pain and immediate swelling.

One of the ee lost his focus of the hazard that exited and reached thru an open valve to clean out 60 me accumulated flynsh. He received a fracture to the right thumb when the air operated value closed unexpected.

EE NOTIFIED COMPANY THAT THE BLOOD-CLOT WAS RELATED TO INJURY ON 11/01/05.
Employee suffered a lower back muscle spasm while carrying an 85 lb. return roller to the tail pully area of belt #3, a 60 inch conveyor. It requires climbing 2 flights of stairs to get to the tail pully of belt #3
While bolting in panel, the employee started to feel pain in his right shoulder. He is scheduled to seek medical attention on 10/19/05. He had surgery on 12/15/05.

The employee was welding inside the primary crusher when he got a piece of welding slag in his eye. Employee did not seek medical treatment until 10/17. He was off work 10/18 & 19.

Putting 6"" water line together. When trying to align the pipe, another employee hit the other end of pipe catching the injured employee's finger.
Lineman was installing used arrestor on power line, when arrestor energized it exploded causing electrical arc. Lineman received 2nd degree burns to upper right arm.
EMPLOYEE WAS SHOVELING AND BENT OVER TO PICK UP A ROCK WHEN HE FELT A SHARP PAIN IN HIS HIS BACK.
Employee was struck by a cable rope while pulling tension on the cable. They were pulling panline chain off the face onto a chain car. Cause of this incident was not moving away from the cable prior to tensioning it up. Compliance with rules was a factor, protective equip not a factor, miner attitude a factor, mining systems a factor.
Fellow employee was tightening a crib with a 3 lb hammer. Hammer slipped out of his hand striking injured in cheat causing a bruise.
Employee was pulling mining cable to make better roadways when he experienced sharp pain in lower back.
Employee was changing out bearing on slope take-up and felt pain in his chest. Pulled muscle in chest.
Employee sustained a laceration to left palm from a piece of stainless steel.
The employee stated he was in the last open cross cut between the no. 2 & 3 entry, on the company no. 004. continuous miner section. He was in the process of walking by the continuous miner in route to get his scoop, when a piece of draw rock suddenly fell striking him on the head, back and left knee area. The piece of draw rock measured approx. 1'x2.5'x4""thick.
Walking past shield #88, tripped, fell against jack, causing injury to his back and rib area. *Employee began to lose time 10/25/05 due to back injury.

While unloading a miner pot off a jeep onto a scoop, the employee pinched his left little finger between the pot and scoop bucket EE was removing the chain from the pot that was sitting on the bucket when it shift. Diagnosed as a fracture to the tip of the left little finger.
EE STATED HE WAS KNEELING IN CROSS-CUT WHILE SCOOP WAS PASSNG BY WHEN ROCKFELL OUT BETWEEN PINS ON 2-B SUPPLY ROAD CROSS-CUT 48+30 STRIKING HIM ON THE UPPED BACK, NECK AND LEG. HE MISSED STARTING TUESDAY'S SHIFT (OCT. 18, 2005)
EMPLOYEE WAS WASHING HER HAUL-UNIT. SHE WENT TO SHUT OFF THE WATER AND DISTURBED A SWARM OF BEES. AS SHE TURNED TO GET AWAY, SHE TRIPPED. AS SHE WAS TRYING TO KEEP FROM FALLING, SHE HURT HER BACK.
Toe was under edge of low boy trailer while unhooking chains. Trailer came down on foot, crushing toes.
WHILE TRAVELING IN MANBUS AS A PASSENGER, THE OPERATOR SPPLIED THE BRAKES AND THE MANBUS SLID CAUSING THE MANBUS TO SLIDE INTO A PATRKED TRAM, RESULTING IN AN INJURY TO LOWER BACK.
Employee reached down to pick up bag to weigh and felt a sharp pain in his lower back.
The employee was sent to move and set a pony belt drive on the construction section. He instructed the crew to hook a 5/16"" chain around the drive and the scoop rope and hook to the chain and tighten up. He was standing beside the winch when the chain broke and struck the employee.
Laborer was loading his truck. He was climbing down the ladder from the bin he was loading from and hit his elbow on the cage around the ladder. The employee did not report the incident or see a doctor until 12/19/05. The office was not aware that this injury occurred at a mine until 1/6/05.
EMPLOYEE WAS MOUNTING THE 450 JOHN DEERE TRACK HOE. AS HE WAS CLIMBING UP HE STRUCK HIS HEAD ON THE OUTSIDE MIRROR BRACKET. HE JAMMED HIS NECK CAUSING NUMBNESS IN BOTH HANDS & RIGHT ARM.
Working in belt slope putting rock on belt line slipped and fell and slide on loose rock and strained left hip and left knee.

TAKEN TO HOSPITAL, X-RAYS NEGATIVE & NO DIAGNOSIS COULD BE GIVEN. 10/15 MRI RESULTS SHOWED 2 HERNIATED DISCS & 1 BULGING DISC. SAFETY WAS NOTIFIED OF MRI RESULTS ON 10/21/2005
Employee was operating shuttle car on uneven bottom when he trammed over bottom roll at fast pace car bottomed out slamming employee into seat. He sustained compression to back.
OPERATOR WAS DESCENDING FROM 980F WHEEL LOADER & MISSED THE BOTTOM STEP ON THE LADDER. SHE FELL BACKWARDS ONTO THE CONCRETE, HITTING HER HEAD.

Employee was hanging monorail along the 18 west LW section belt. As he pushed up on the monorail, he felt pain in his lower back. First day missed due to injury was 10/25/05.
Electrician was removing and screw from a foot switch when screw driver slipped off screw and punctured left hand.
7I SECTION, EMPLOYEE WAS BACKING REMOTE MINER UP WHEN HE STEPPED IN A WATER HOLE, TWISTING HIS LOWER BACK. EMPLOYEE ALSO HAS HAD PRIOR BACK PROBLEMS. LOST WORK DAYS BEGAN 10/24/2005
THE INJURED EXPERIENCED SORENESS IN HIS BACK AFTER OPERATING THE PC400LC-7 KOMATSU AFTER DIGGING OUT LARGE ROCKS AND THE BUCKET SLIPPED OFF SEVERAL TIMES DROPPING, CAUSING JAR SENT TO THE CAB AND SEAT.
WALKING UNDER PLANT, SUNK DOWN INTO SAND AND STRAINED LEFT KNEE.
EE was prying on a mantrip arm bracket with a slate bar. He had bent the bracket while trying to install a bushing that he made. He used a pry bar to straighten the bracket. The pry bar slipped and the sudden movement of the bar strained his shoulder. He sought medical care on 10/28. On 12/12 he began missing time for surgery on his shoulder to repair a rotator cuff tear.
WORKER WAS CLEANING THE TROUGH ON THE KILN ROOF. ROOF GAVE WAY AND THE WORKER FELL APPROXIMATELY 30 FEET TO THE GROUND. WORKER SUSTAINED BROKEN AND DISLOCATED FEMUR AND THREE BROKEN LUMBAR.
Operating D-10 dozer on the fill. He had turned his front lights and fender lights off to dismount machine. While dismounting he missed grabbing the handrail located on the deadlink causing him to fall backward pinning his left lower leg and foot between deadlink and pusharm causing him to fall to the ground. X-rays were negative. On 10-14-05, injury became L.T.A. Poss.MCL injury.
The employee fell from the bucket of a skid steer loader, he bruised his right hip and his right leg and his right knee.
EMPLOYEE WAS BUILDING A BRATTICE WALL & AS HE BENT OVER TO PICK UP A CONCRETE BLOCK, A PIECE OF ROCK FELL OUT FROM BETWEEN THE ROOF BOLTS STRIKING HIM ON THE HEAD, NECK AND BACK.
Employee had been operating a underground tractor watering travelways. He had backed the water car into an area off of a intersection to begin filling it. When he went to exit the cab he started having muscle spasms in his lower back. He stated there wasn't any certain incident that caused this.
Injured was lifting box of resin and strained back.
Employee was riding in a 5 passenger personnel carrier along with the operator. They had departed the motor bern traveling toward the south portal back switch. Upon reaching the switch they collided with another carrier resulting in employee's fingers getting smashed.
EMPLOYEE WAS PINNING TOP, DRILL BIT BROKE. DRILL STARTED SPINNING, STRAINING RIGHT SHOULDER. STARTED LOSING TIME (OFF) 10-31-2005.
Cleaning up belt line with scoop, got too close to rib rock rolled out and hit him.
Alleged incident was to have occurred while this employee was installing a bolt into the roof. He was bending it straight when he claims his shoulder started to hurt. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equipment not a factor, miner attitude a factor, mining equip/systems a factor (One piece 8-foot bolt).
Employee was bolting, putting in 6' bolts when while pushing up a bolt it got stuck. While trying harder to push it up, his left shoulder popped, causing pain and swelling
TAKING STOPPING OUT, BLOCK FELL FROM TOP OF STOPPING, MASHING RING & LITTLE FINGER ON HIS RT. HAND. LITTLE FINGER REQUIRED STITCHES.
While walking to stairs of Gyro area, EE stepped over a concrete foundation. He caught his right foot and tripped. He tried to catch himself and fractured his left wrist and cracked his right elbow. Began losing time 10-12-05.
Employee was helping build an overcast. He was on an elevated platform placing patio blocks on the top of the overcast. He complained of pain in his upper back and left shoulder.
LOADING CONVEYOR BELT ON TRAILER WITH CAT 988 LOADER. SLIDING CONVEYOR UP TO ANOTHER CONVEYOR ON TRAILER. EMPLOYEE BROKE HIS ANKLE BECAUSE HE HAD ONE LEG PLACED BETWEEN THE CONVEYORS WHEN THEY WERE SLID TOGETHER.
CHANGING BLOW BARS, GETTING READY TO REMOVE LAST ONE ON BAR, PULLED PIN TO ROTATE, EMPLOYEE SLIPPED ON OLD BAR & LEG WENT BETWEEN BLOW BAR & CURTAIN PLATE.
Employee was installing a 6' double lock bolt, when he tried to push bolt into roof he felt pain in mid back.
EMPLOYEE WAS MAKING A SPLICE IN CABLE WHEN HE HIT HIS LEFT THUMB WITH A HAMMER.
Employee was carrying 2 buckets of miner bits and hung his foot in loop of miner cable causing him to fall and hurt left knee.
Employee was climbing an extension ladder to secure & bolt up but the ladder slid out from underneath him before he reached the top. Right shoulder dislocated when he attempted to hold himself from falling 4 ft to ground.
WHILE BOLTING, A PIECE OF COAL (12"" X 16"" X 3"") FELL FROM THE ROOF AND HIT THE EMPLOYEE'S RIGHT SHOULDER.
EE WAS CARRYING A ROCK, FELL ON HIS KNEE AND TWISTED IT.
I sent welders out back to pit to flip teeth on dragline bucket. All you need is a torch and hammer. after you cut find out of teeth you have to knock them off but they cut one off and it fell off and EE's foot was up under it.
Employee driving loader on levee not fully formed. The levee collapsed turning loader over on side. EMS was called but employee refused treatment stating he was fine. He worked the remainder of that week and took vacation the following week. On 10/24 he missed work saying that he was indeed injured. At this point the incident became a lost time accident and then became reportable.
Working on disconnect knives of a motor starter bucket in a motor control center. Power was on. Employee inadvertently placed screwdriver on energized knife and door saftey catch of motor control bucket. Injury was arc flash burn to right and left hand and face. The said bucket was on the bottom and employee was laying on left side during the repair.
Employee strained back and right shoulder while drilling.
Dump area was getting undercut. Loader operator warned truck driver about situation and no response. Next load brought up, he backed up too far and the pile gave way and the truck slid down the material and flipped over onto its top. Truck operator was not wearing a seatbelt.
Employee was bolting top when the drill pot hung on a bolt & dropped on his knee (R).
Employee alleges he was drilling a hole, the drill slid down and he rolled out of the cab onto his back.
INJURED AND ANOTHER PERSON REPLACED FRONT WHEEL ON FRONT WHEEL ASSIST TRACTOR. AFTER TIGHTENING WHEEL BOLTS THEY LOADED 6 IN X 8 FT WOOD POSTS IN BACK OF PICKUP. WE ARE NOT SURE WHICH OF THESE ACTIONS CAUSED HIM TO STRAIN HIS LOWER BACK OR BOTH DID.
INJURED WAS WORKING ON THE P5 BOLTER ON #3 UNIT WHEN A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK INJURED KNOCKING HIM INTO THE BOLTER CAUSING AN ABRASION/SCRATCH AND FRACTURE ON HIS LEFT CHEEK.
Was working on plant. came down steps stepped on ground into low area where rain washed out small area at foot of steps. Turned ankle.
EE alleges that he was shocked while leaning against electrical box. Witness saw employee fall but cause of fall is still under investigation.
EE was making a connection on a conveyor truss walkway. A weldment approx. 4' from He failed resulting in a collapse on which he was standing. He fell approx. 6' and caught himself on a structural member of the same truss resulting in injury to his rib cage.
Employee was changing a tire on a truck. He lifted the flat, muddy tire to place it in the back of the truck, when he felt strained in the back and groin area. The condition worsened as the day when on.
Battery of crusher caught on fire; employee lifted off cover of battery to aleviate fire when battery exploded causing battery acid to burn eyes and face; pieces of battery cut employee's face.
Employee was loading pins and plates on top of roof bolter and a bundle of pin plates fell striking his right foot.
Member was attempting to hang his buggy anchor, he was located on the left side of the feeder on the #2 section, as he was hooking the chain over teh anchor bolt with his other hand, he felt a sharp pain in his lower back. X-rays were taken and were negative but he was advised to stay off work until 10/17/05.
EE TRAVELED INTO UNSCALED AREA & WAS STANDING AT FACE. HE PULLED LOOSE ROCK FROM FACE BY HAND. ROCK WEIGHED 112 LBS. ROCK HIT EE ON 1) HEAD: HIT HELMET & R. SIDE OF HIS HEAD WAS SCRAPED, BUT NO STITCHES REQUIRED, 2) L. MIDDLE FINGER: CUT & NAIL TORN OFF. SURGICAL REPAIR & PT REQUIRED (NOT AMPUTATION), 3) R. FOOT: FRACTURED. EE IGNORED TRAINING TO STAY OUT OF UNSCALED AREAS.
EE was attempting to install an oxygen tank guard tube measuring 11"" o d pipe w/1/2"" thick walls x 34"" long on the #5 mainten. truck. EE had it supported w/a 10"" long piece of 6"" channel, channel iron slipped out from under the pipe before the ee could tack weld it in place causing the channel iron to strike him on the rgt thumb & index finger and hyper extended his lft thumb.
Alleged incident was to have occurred while this employee was bending wire mesh roof screen to throw away. He claims his back went out while doing this job. Cause of this injury was proper lifting techniques. Compliance with rules/regs is not a factor, protective equipment not a factor, miner proficiency a factor, mining equip/systems not a factor.
EMPLOYEE WAS STACKING 100 LB BAGS. NOTICED SORENESS IN RIGHT WRIST TOWARDS END OF THE SHIFT. WENT HOME FOR THE EVENING AND WOKE UP WITH SWELLING IN THE MORNING. NOTIFIED SUPERVISOR AND SAW DOCTOR FOR X-RAY. EMPLOYEE WAS GIVEN RESTRICTION OF LIFTING IN EXCESS OF 10 LBS FOR ONE WEEK A BRACE IS ALSO TO BE WORN FOR ONE WEEK.
CONCRETE PUMP WAS BEING CLEANED OUT AFTER A CONCRETE POUR. THE EMPLOYEE WALKED IN FRONT OF THE DISCHARGE WHILE RETRIEVING A SHOVEL, AS CONCRETE WAS BLOWN OUT. DEBRIS HIT HIM IN THE LEG.
Employee didn't miss any time away from work. Not aware of injury until MRI was done on 10/25. Employee was supervising roof bolting activity. Told bolter operator to back out of area because he didn't like the ground conditions. Piece of rock approx. 4 to 5 ft. broke off striking employee on head, right shoulder and lower back knocking him to ground.
Employee was using a hammer to help open a sump gate on Sag 2 sump when he felt a pop in his lower back above his left hip. He continued to work but later after using the skid loader, he felt tingling in his toes. He was taken to the dr. given a prescription and taken off work. Currently on going.
SETUP AT NEW LOCATION. HE WAS LOOSENING THE BOLT THAT ALLOWS THE JACK LEG TO SWING DOWN. WHILE PULLING ON THE WRENCH THE FORCE OF IT BRAKING LOOSE CAUSED HIM TO STEP BACK & HIT HIS SHOULDER ON THE GUARD BEHIND HIM. AT FIRST HE HAD PAIN & THOUGHT IT WOULD IMPROVE BUT HE HAS DEVELOPED MORE PAIN & NUMBNESS & NOW NEEDS MEDICAL ATTN.
Employee was operating a battery forklift near the end of the track unloading rockdust and contacted the mine roof with his left ear and side of head while backing up. This resulted in a laceration to his left ear that required 12 sutures.
