A fall of roof occurrred in the 4K Panel at x-cut #11, spad 6+95, between # 8 and #9 entry. The fall was approximately 30ft long x 19ft wide x 8ft high.
a unintentional roof fall , #2 unit beltline crosscut #6, length 50' , width 18' , thickness 5' This area was pinned with 72'' tension rebar roof bolts.
The dayshift electrician called control room operator to report fire near control room. The control room operator and shift foreman discovered the department pick-up truck on fire in parking lot behind control room. There was no driver in truck and had not been driven during the shift. The fire burned for approximately 20-25 minutes before being extinguished by fire department.
Roof fall - main north at spad #4100, intersection #6 (belt entry) approx 19'wide X 19'long X 9'high
The unintentional roof fall was 40' long x 18'3"" wide and 8' high. It occurred in the #7 entry at spad # 1507.
A fall of roof occurred in the 1st Northwest Mains at x-cut 164 in the # 5 Entry. The fall was approximately 19ft long x 18ft wide x 6-8ft high.
A fall of roof was discovered in the 1st NW mains at x-cut 155 between the #3 and #4 entry. The fall was approximately 16ft long x 19ft wide x 6-8 ft high. This fall was previously reported on e-doc #1132254 on 6/2/10 per a discussion held with D10 Roof control specialist it was determined that (2) separate roof falls could not be recorded on the same 7000-1
Service power from the power company was interrupted during a storm. Power was restored a short time later.
ONe miner was trapped for 45 minutes on the Sugar Run elevator due to the door switch needing adjusted.
A possible methane ignition occurred on 002-#1 section in # 5 entry 60' inby SS# 15043 during the first left lift off of #4 pillar block. MSHA and DMME were notified 103j order was issued by phone and modified to a 103k upon arrival. No visible evidence of ignition on rib or Miner.
When our man hoist was switched from generator power to line power a breaker tripped and the hoist was down for more than 30 minutes. There was no damage to the hoist the breaker was reset, the hoist tested and it was OK. No one was stuck in the shaft.
Section of the roof gave way in an intersection at the first break inside the mine. It was about 5' thick X 18' w X 20' L. This mine is just getting first line of breaks cut through.
Due to a problem incurred somewhere along the transmission lines of the company that provides electricity to the mine, electricity to the mine was lost. This resulted in the main hoist being inoperable until electricity was restored.
The B80 Bio Diesel fuel transfer line to the underground mine ruptured in the production shaft. This resulted in a fuel spill of 1500 gallons of fuel and forcing an evacuation of the mine due to slippage of the production friction hoist.
The Kuhntown elevator went down at 9:15 p.m. with 2 persons on board. The 2 people were removed safely from the elevator at 9:49 p.m. the problem with the elevator was that the contactors did not make-up.
A roof fall was discovered by Mine Foreman while re-routing life-line.
An unplanned roof fall above the installed roof bolts. The fall is located at #18 cross cut along the #1 belt between #2 and #3 entrys. No one was injured, trapped and no equipment was involved. The fall was an extension of a fall that was reported 6/22/09.
During an inspection by an MSHA inspector, a roof fall was found in the return of the Southeast Submain at crosscut #36 in entry #3. The fall was located in the route that an examiner would travel during his weekly examination of the area.
Reportable roof fall in the tailgate of the LW. No employees were involved in the fall. About 30 feet outby the face in the tailgate entry. The roof mesh failed and the immediate roof fell about 4 feet high between the can cribs and the barrier pillar for a distance of about 20 feet. Passage was possible but not advisable between the can cribs and the solid mining block.
A fall of the mine roof has occurred at #8 brk in the #6 entry along #3 south beltline. The fall was approx 50' L X 20' W X 12' thick. The area has been cribbed and will not be cleaned up and does not impede travel or ventilation.
The Kuhntown Elevator went down at 7:05 am with one person inside the elevator. At 12:00 pm the individual was safely removed from the elevator. At 1:55 pm the elevator was fully operational after the run-by on the govenor counterweight was adjusted and the elevator was determined to be safe to return to service.
A roof fall was discovered by Mine Foreman and shift foreman while traveling Intake. No interuption in ventilation and no equipment involved.
A roof fall measuring 20 feet by 20 feet by 7 feet was found in the intersection of the # 5 entry at the # 26 crosscut of the 4 right panel. There was no change in the mine's ventilation.
The Kuhntown elevator went down at 8:20a.m. with no persons on board. The elevator was back in service at 12:00 p.m. The problem was electrical,it had a burned resistor.
On this date the service power tripped. The 8' hoist was down due to ground control(Scales)problems. We had trouble rebooting the 14' hoist computer. This left the 16' shaft the only shaft operating. An evacuation was started. As the evacuation was underway, the 14' hoist was rebooted and in service. The call was made to MSHA during this process.
No injuries. Roof fall on the west side of the mine and the primary escape, at 90 E impeded travel. 10 miners were in the mine, zero contractors.
A fall of the roof occurred in the NE mains at x-cut 208 in the on belt. No personnel, equipment nor ventilation were affected.
A roof fall was discovered at approximatley 12:30 p.m. on 5-24-10. The fall was approximatley 20' long x 20' wide and 6' high. Exact location of the fall is: # 8 entry, #2 zone , #62 block crosscut 1 crosscut inby spad #447. Mine Foreman discovered the fall.
A unintentional roof fall occurred at this location. Approx. 8000 ft off#1 Section, SS# 312, Left return. No ventilation was blocked, no escape way was blocked. Fall was approx. 20 wide 20 long and 8 to 10 high and will be cribbed off and not cleaned up. Fall was found during weekly examination. This area was mined 2/2003 and supported with 5 torque tension bolts.
An elevator was stalled for approximately 2 hours, 50 minutes.
A fall of the roof occurred in the 1st SE Sub Mains x-cut 5 between #2 and #3 entries. The measurements of the fall are 55' to 65' in lenght X 19' wide X 5' to 7' high.
Employee was contouring turn in road and hit a 6"" natural gas line.
Loading crew was cutting coal in the no..1 face on the no..8 section in the middle push of a three push system when the ignition occurred while cutting bottom. The flame was orange and blue in color and lasted approximately 30 seconds, one fire extinguisher was used to extinguish the ignition
A rock fall was discovered outby the 001-0 section in the #4 entry. The fall did not affect persons, ventilation nor equipment. The fall was approximately 12'high X 30'long X 20'wide. The escapeway was rerouted and all miners informed of the new location. The fall was dangered and cribbed off.
A roof fall was discovered in the number 6 entry, of the 034/036-0 MMU last open crosscut. Estimated size of roof fall is 8 foot high, 16 foot wide, and 14 foot long. No injuries were reported as a result of this roof fall.
A roof fall appx. 30'x19'x6'8"" was found on 5-19-2010 in the #6 entry at spad # 810 appx. 2600' from the active face. The roof in this area is mixed and is supported by fully grouted resin bolts and 8"" cable bolts. The fall did not affect ventilation nor was any equipment or miners affected by the fall.
A roof fall measuring 30L x 15W 8H occurred 100 inby survey station 1719 in the No. 2 SW Mains off of the the No. 1 SW Mains. The area was bolted with 5 TT bolts.
EE was traveling with MSHA inspector, walking Alloy's return airways to MP's #22 and 23 when they encountered a roof fall at spad # 1753, an intersection in a adjacent entry to the entry the airway examiner normally travels. These air ways are all 'common' in a 9 entry panel. We dangered the area off.
1400 level, 21N North face had several small flames after blasting. After checking CO and methane levels, supervision extinguised the flames.
EE was traveling with MSHA Inspector, walking the alley main return towards the return air shaft - when they encountered a roof fall @ block #5 - spud #258 - a intersection in a adjacent entry to the entry the air way examiner normally travels - These air ways are all common in a 5 entry panel - we dangered area off.
Initial report of individual trapped on grapevine elevator for 30 minutes prompted call to MSHA hotline. Upon resetting of the elevator by employee on surface it was revealed by the elevator time stamp that the individual was entrapped for 25 minutes which is not a reportable incident. No damage to elevator. Report being submitted to expire Hotline Call.
Employee was traveling with MSHA inspector, traveling Alloy's return airways to MP # 22 & 23 when they encountered a roof fall @ spad # 1602, an intersection in a adjacent entry to the entry the airway examiner normally travels. Thee airwys are all 'common' in a 9 entry panel. Dangered area off.
Brake car brakes set because of low voltage, causing brake car to be unavailable for secondary escapeway puposes. Mine was evacuated.
A roof occurred in the 1st West sub-main, entry #9, spad # 8+40. The fall measured 25"" long X 19' Wide X 8' high.
The shear was cutting coal & rock going to the tailgate when the ignition occurred on the headgate drum. Ignition happened at 60 shield and lasted approximately 2 to 3 minutes, the flame was put out with two fire extinguishers. The longwall crew was headed to the tailgate on their third pass of the day.
A roof fall was discovered in the 3rd Northwest main between xc-17 and xc-18 and between spad #12+60 and 11+90 in the #5 entry. The size of the fall was length-15ft, width-19ft and height-4ft.
The miner was cutting in the no.2 face on 8 section when the ignition occurred. The miner had cut approx. 9 foot out of the face on the curtain side, the operator had sheared down with the miner head to cut the mine floor and as soon as the miner head touched the mine floor, the ignition occurred. Ignition was put out with one extinguisher and water.
Roof fall , #1 unit , 2nd panel, #6 entry , between #11 and #12 cross cut. 50'long, 18' wide, 8'high. (MSHA was contacted)
An unintentional roof fall occurred off the no.2 section in the no. 5 neutral entry at spad no. 4544. The fall measured 20' long, 20' wide and 8' high. This fall did not affect the travel of men or the flow of air.
A roof fall was discovered in the 2nd Northwest submain xc-27, spad #19+22 in the #2 entry. The size of the fall was length-25ft, width-19ft and height-6ft.
Ignition- The continuous miner CM81 was loading the cleanup buggy with coal in the no.2 to no.3 x-cut on the off curtain side of the face, when the methane ignition occurred on the no.8 section. The ignition was extinguished with two fire extinguishers.
A fall of roof occurred in the South East Submain at # 28 cross cut, # 3 entry. The fall measured approximately 30 feet in length x 30 feet in width x 6 feet thick. The area was supported with 6 foot fully grouted roof bolts.
While mining with the continuous miner, an old abandoned mine was cut into, releasing water.
A roof fall above the anchorage of the bolts was discovered by the mine examiner. The fall measured 70' long X 19' wide X 6' high. at spad # 2865 in the #1 entry.
Hoist operator lost power to hydraulic controls due to a burnt up hydraulic electrical control switch. The control switch was replaced and the hoist was operational at 11:00 pm.
At approximately 1:35pm the elevator stopped approximately 30 feet from the top landing with two employees on the elevator. The elevator was examined and the cause was found to be the top landing enter lock door switch. The switch was replaced and the elevator was put back in service at approximately 2:07pm.
While traveling to start a belt that was down, the section foreman noticed a fall had occured. This was in the 1st southwest mains belt entry, two crosscuts inby spad 1836. Approx 20' wide x 30' long x 9"" high. The area was cribbed, timbered and bolted. All accesses to the fall were dangered off. Vent tubing was humg to divert gas to return.
A roof fall was discovered in the 3 North 4 Mains, 85 crosscut, in #8 entry. The roof fall measures approximately 20' long, 18' wide, and 6' thick.
#3 hoist tripped on vibration fault. Upon inspection it was determined that one of the hoist ropes had a visible defect. Although it was not determined whether rope met the retirement criteria under 57.19024, a decision was made to change one of the hoist ropes. There were still 2 escape ways.
Production Hoist tripped when 1 phase of 4160V feed was lost near the FC building. Cleveland Public Power was called and they repaired the 4160 V feed. Test trips were run and employees were sent back into the mine.
During the quarterly inspection, MSHA inspectors requested to exit the mine through the escape shaft. When decending the skip to the bottom, the skip would get stuck prior to reaching the bottom. Upon investigation it was discovered that a bulge had developed at approximately the 350' level of the shaft.
200 feet inby survey spad 27346 on MMU 006-0, a 10 foot long by 8 foot wide section of mine roof fell above the anchorage point.
A roof fall was found in Old 9 Mains, in the #4 entry between brk. #23 thru brk.# 26. The fall was approx. 5 'thick, 20' wide, 180' long. The area has been dangered off. No ventilation or machinery effected.
The 5-north air shaft hoist went down at 9:15am (mining ceased at this time) Hoist was being tested at this time. The hoist had a low oil switch malfunction. Hoist was repaired at 11:15am mining resumed at this time.
A roof fall measuring 8' x 19' x 19' occurred on the Jenny Branch section near spad No. 5095. *The fall will not be cleaned up.
An unintentional roof fall occurred on 5/11/10. The fall occurred on #3 belt mains, #5 entry, Bk #10, spad #1277. No injuries occurred from the fall (length 30'-60', width 10', Thickness 6' plus - above anchorage) (support 6' 3/4"" Torque tension, 10' cable volts, and cribs).
Roof fall discovered on 2A Bleeder Panel at SS# 825 # 3 entry by Ronnie Maggard. Size of fall 40' long x 10' high. Area is timbered off. Fall does not affect ventilation or travel. Fall discovered at 9:05 AM. Fall will not be cleaned up.
Cross timbers from substation pole caught fire. It was a windy morning and some of the embers from the pole blew into a field creating a grass fire.
Had a continuation of fall in #3 entry that occurred 11/2/09. There were no injuries. Roof is heavily laminated in this area. Additional cribs were built in surrounding areas. Fall was inspected by MSHA and by State Offices.
A roof fall was discovered in the 3H panel #7 entry between xc-13 and xc-14 spad # 8+25 - 8+95. The fall was approximately 40ft long, 19ft wide, and 8ft high.
Pop off valve on the air tank, located in the #1 Hoist Pit failed.

Roof fall off section at the end of 1st West Mains of the 002 Section in the #3 entry.
A roof fall occurred on the 2 south track one x-cut inby spad #9182. The fall was about 2 to 3 foot high, 10 foot wide and 15 foot long. The area will be cleaned up and rebolted.
Cleaning had been conducted on top and bottom of slope, it is believed that debris became lodged in the pulley of the elevator ropes. This allowed the ropes to become entangled and made the elevator inoperable.
A methane ignition occurred on the 17-w section #3 entry at 6:15 am on 5/8/10. The section was placed on a J order and then modified to a K order to allow temporary roof supports to be installed so packers could be installed on both sides of the vertical-horizontal de-gas holes (well BH-3A st 2).
On 5/8/10 the 17 W. section #3 entry experienced an inundation of Methane in excess of 5.%. After intersecting horizontal WCC (BH-3A-ST-2) the power was removed immediately from this area as per cut thru plan. Entry was re-ventilated to correct condition. Section was placed on a J order, then K order to allow mining to resume.
Roof Fall in worked out area of mine due to pressure from roof.
The following falls were discovered in th 9th SW Mains Intake in the #2 entry. From x-cut 25 to 27 it was (150ft l x 20 ft w x 5 ft to 6 ft). At x-cut 42 to 48 it was (560 ft L x 20 ft W x 5 ft to 6 ft H) and from x-cut 62 to 64 (140 L x 20 W x 5 ft to 6 ft H). This area was developed 9-30-07. This form is being submitted to abate citation # 8498443 issued on 5/6/10.
Hoist would not work when called. Power pack went out on hoist and was replaced. The hoist was down from 12:25 am to 1:18 am.
We believe starter area of dozer shorted out. Cab burnt, back part of engine compartment, and pumps below cab burnt. Burnt beyond repair.
A fall of roof occurred in the 4B Panel at x-cut #36 in the #6 Entry. The fall was approximately 20ft long x 20ft wide x 8ft high. The cause of the fall was due to water coming from the top.
Roof fall was at #3 x-cut on 2 left west mains. Fall was 18'w x18'L x 8' to 9' thick. Two cribs were set at all approaches to fall. Area had been bolted with 5' torque tension bolts. and spotted with 8' bolts. Fall was in neutral 1st x-cut outby spad # 339. Area was last mined October of 2009.
An unintentional roof fall occurred at the Galatia Mine New Era Portal on the 6th West Tailgate #4 Entry set-up room from 730' to 790' mark. The fall was approx. 60' long, approx. 24' wide and approx. 12' thick. The fall did not affect ventilation or passage of workers. The fall will be cleaned up. Fall # 27 F.
Water inundation. The water entered through the bleeder system in the A-side return.
An employee was unloading supplies and left the skip door too low. As he attempted to grab a pallet with the fork lift the mast of the forklift hit the door causing it to get jammed. This was on the 14' Skip.
Approximately 7:am Owl shift reported vibration on the service elevator as they was comming out. The elevator was examined during the morning hours and nothing was found. At approximately 12:10 pm the power was removed from the elevator to change out some control boards and make adjustment to the pods. Power was restored at approximately 2:30pm.
Water run off due to heavy rains in the area caused the water retention areas in the mine to rise. As a precautionary measure the mine was evacuated because water on level 2 of the mine was closed to the base of level 1. two MCC and transformers. There was no immediate danger. MSHA was notified.
A roof fall approximately 20'x100'x5', 1300' outby active face occurred at 1:52 PM. There were no injuries or equipment involved in the fall. The area will be rehabilitated.
Blower motor for hoist drive shorted out. The other two hoist were functioning at this time.
The #1 elevator was out of service for a period of time exceeding 30 minutes. Water had accumulated in the pit causing the comp chains to move. Movement of the chains caused damaged to the buffer switch. The switch was replaced, the water removed and the elevator was placed back into service @ 11:13pm. No one was on the elevator at the time.
During a routine inspection of the Mine Air/escape shaft hoist, it failed to energize for additional testing. After contacting an electrician, diagnosis showed a stop relay switch failed to reset after the cage landed at the surface.
The compressor that supplies air to the brakes of the 8' Hoist overheated. Although the hoist has a back-up nitrogen system for the brakes of the hoist. We shut the hoist down and and cleaned the coils of the compressor to get it back online.
A fall of roof occurred in the Northwest Submains in the # 3 Entry at cross cut 27 spad 18 + 90. The fall was approximately 30ft long x 19ft wide x 8ft high.
An unintentional roof fall occurred at the Galatia Mine New Future Portal at the No6 seam entry #3 1360 So. And 1475 So. The fall was approximately 25 feet long approximately 18 feet wide and approximately 8 feet thick. The fall did not affect ventilation. The fall will be cleaned up.
The operator of the water truck miss shifted causing him to lose control, and allowed the truck to roll backward and up the bank. This caused the truck to roll over. The operator was wearing his seatbelt and was not injured.
The Kuhntown elevator went down on 5/1/10 at 7:10am with 1 person on board. The person was safely removed at 5:55pm. The counter weigh bearing was bad the repairs were completed on 5/4/10 at 8:15am. The elevator was inspected and released for normal use buy MSHA and state.
Dozer pushing in shot, blade contacted undetonated booster resulting in explosion in 1-5 Pit Tip Top.
2 miners lit their fuse for a face round and then re-entered blast area prior to the mandated 1 hour wait period. When they were within 25 feet of the face, a misfire detonated. This was against company policy and recent training. 1 miner was killed by the blast and the other injured.
Blasted drift and was ready for baring down and mucking. Unknown cause of death at this time. The victim was hand scaling the north rib of the left 4500 drift when loose material fell on him.
Employee was working on a Raymond Roller Mill when he entered into the roller area of the mill. The mill was energized and the rollers crushed him.
EE was cutting drill head from drill steel. Cutting operation resulted in fragmentation of drill steel. EE was struck by drill steel fragment resulting in his death.
Was loading rock out of #7 face. The shuttle car was making the turn out of the entry. EE was caught between the shuttle car and the rib.
Drill operator was fatally injured by the drill rig he was operating. The likely scenario is that the drill rig began to roll forward while he was at the back of the rig operating the controls for the drill. The victim may have attempted to stop the drill rig and was caught up under the rig and suffered fatal injuries. There were no witnesses to the accident.
EE was operating the longwall shearer and a piece of rock was thrown from the cutter drum and struck him in the left hand amputating part of his index finger.
EE cleaned top of chute with short scraper, change to long scraper for bottom. Scraper contacted rotary feeder, ee right hand pull against steel grated on chute. EE transported to ER, exam revealed fracture to right hand and lower RT arm. Partial amputation right little finger end.
Employee was bolting and let the head down catching his finger. The tip of his middle finger was cut off of his left hand.
While rotating a stripper rod, the rod dropped 3"", resulting in a crushing amputation of the tip of the employee's right pinky finger.
EE stated that he was hanging his tie off to a bolt anchored in the roof. He twisted his body when he reached up to hang the shuttle car cable tie off, he felt pain in his neck and right shoulder.
Injured was walking along South Mains B Belt stepped on the edge of a small hole rolling left ankle. PPE was not a factor, compliance rules/regs not a factor, miner profiency was a factor.
Injured employee felt a pronounced bite to his stomach region. The injured employee was transported to the local ER & was teated for a mild case of anaphalactic shock, sent home with a medical release for the following evening. The injured employee missed his first scheduled shift back to work.
The injured employee was removing boom canopy from Fletcher bolter when it fell off striking him on the right lower leg causing a fracture to his tibia. he was transported by ambulance to local hospital.
Employee was preparing to bolt # 5 Heading. He was on his knees cleaning out his dust box when a large section of the rib broke off and fell striking the employee on the back of his legs and feet.
Moving over to put up bolt and caught foot between two pieces of rock. Strain to knee.
Employee was pulling up slack in bolter cable while on his knees and twisted his back
Employee was working on stopping in the return when a piece of rock fell off of rib hitting employees left foot, employee became a lost time on 6/1/10 - 6/7/10 with bruised left foot. Employee has been released to reg. duty with no restrictions.
EE was loading a flatbed trailer. EE placed his hand on the forklift mast and crushed three fingers when the forklift operator moved the mast.
Operator fell off top of operator's cab of drill onto his back. He fell approx 8' to the ground. He was assisting mechanic, as steel had come out and they were attempting to push back in . EE has broken rib and lung was punctured. Also broken shoulder.
Employee was completing a double front tire rotation on haul truck 415 he lowered the jack on the right front bumper, then moved to lower the jack under the front A-arm assembly. As he started to lower jack, it slid out under pressure striking employee in the right side of torso. Right arm contusion with right ankle sprain.
EE noticed the water pump was not pumping water. EE checked the pump to find out it had lost it's prime. The EE placed his hand on the discharge hose when the hose came off and hot water and steam covered the EE burning his chest and thighs.
EE was walking down the belt entry when his left shoulder hit a belt hanger causing him to fall backwards onto his SR100 SCSR. EE continued to work until 06-17-2010. Due to continued pain he visited Med Express and was instructed to remain off work.
The operator dumped at the bottom of the stock pile all day the previous day.The next day the driver had dumped a load in the middle and admitted to overcoming the berm and the truck went over. The driver exited the truck on his own power.
Building Brattice & noticed sharp pain in lower left abdomen. Did not start missing work until 6/4/10.

While employee was bending over on the walkway of the excavator looking at oil sight glass, employee stood up and we think he felt dizzy and fell from walkway. Employee was taken to the hospital.
The employee had a ladder set up outside the BC108 building. He was almost to the top disconnecting a conduit with a pipe wrench. The pipe wrench slipped and he lost his balance and fell off the ladder. He first hit a snow cover which was below him and then he hit the ground. MSHA lost time accident***05/28/10.
EE was spot bolting near No. 6 belt drive when a piece of rock between two existing roof bolts fell and struck his head and left shoulder.
Employee was energizing the 995 breaker on a generator for a High Wall Miner. The breaker arced and flash burnt his left arm from wrist to his elbow. 99% of the injury was first degree, with a minimum amount of blistering. Upon investigation of the incident, it was found that a loose wire had fell down and crossed over one phase of the breaker which caused the arc.
The employee was helping change out belt rollers. He picked up and handed a top roller to another employee when he reported that he felt a sharp pain in his lower back. This injury was determined on 1/18/2011 by an Independent Medical Examiner to be a new industrial injury. MSHA Lost Time Accident 10/25/2010.
EE pinched his right thumb when placing a cable reel on the ground. Mechanic was on the ground positioning the cable reel with another mechanic operating the boom truck, when the reel came down and pinched his thumb.
EE was using a 3 ton chain come-a-long to gather slack chain being generated by the headgate tensioner when the face conveyor flight that was being held in place by the stops broke. This let tension release back into the face conveyor chain, causing the hook on the chain come-a-long to bend & release striking EE on the left lower leg causing injury.
EE took a lunch break and retrieved his bucket from the first aid car. A rock 3' x 2' x 4"" fell onto his right knee. X-rays were negative but made folow-up with specialist on June 3rd, when he received orthoscopic surgery for a torn meniscus. (two staples)
EE was un-loading crib blocks off of flat car. While carrying crib blocks injured EE stepped on a water line putting his hand in the top striking a roof bolt causing injury to his right hand.
Pulling on water hose as he was cleaning truck scales and strain his shoulder.
Employee pulled his Gob Truck up to a berm. As he dismounted from his truck he slipped on the berm and twisted his lower back. He was seen by a physician on June 1 and did not return to work.
Operator called in to shop that his truck was running hot, the shop foreman told him to check the coolant level in the tank, if it was full he would send a mechanic to fix the problem. The operator decided on his own to remove the radiator cap. He was sprayed on the back with antifreeze and got 1st and 2nd degree burns on his right shoulder.
Employee was welding on a set of air lock doors located near the surface, when later on he discovered that he had burned his right eye.
While cutting a piece of shot wire, employee cut left thumb with knife.
Injured was turning over scoop speed reducer and felt pain in back.
EE stated he and 6 other EE's were reclaiming a belt structure. The EE bent down to remove the roller off of the chain that was securing the load and a co-worker dropped a belt side rail onto his left thumb. This caused a fracture to his left thumb, EE was wearing gloves at the time of the incident.
EE said (the next day, after working the whole shift without any mention of the event) he hit his head on the mine roof. *Please note that this alleged accident is still under investigation.
EE was roof bolting first cut at intersection, slag of rock fell between left rib and operator at #3 left break. EE was hit with the slab of rock causing serious injuries.
Employee working on a 480 volt starter experienced an arc flash blast resulting in burns to his hands and face. He was evaluated by a physician and taken off of work.
The mine received a phone call on 5/27/10 from the employee who stated his neck had pain radiating down through his shoulder. He stated he did not file an accident/illness prior leaving the property. Medical received on 6/3/10 states there is no acute injury but chronic disease.
While unloading a 50 pound bag of rockdust the employee felt pressure on the right side his abdomen which turned out to be a hernia. The employee had surgery to correct the hernia on 8-20-10. This did not become a lost time accident until 8-20-10.
While performing service work on dragline, a co-worker dropped a steel pin weighing approx 10lbs. The pin hit the deck and fell against his knee.
On 5/26/10, the employee twisted his right knee dismounting a dozer. Then, on 6/3/10, the employee's right knee popped while he was mounting a dozer & he was taken to a hospital for treatment. He began missing work on 6/4/10 as a result of these incidents.
Employee stepped onto pump to untangle pull chain. When he stepped off the pump, his foot slipped causing him to fall striking his right side and back on the base of the pump. his doctor removed him from work on 6-1-10.
EE has a Dr who told him his back/neck problems result from working at quarry. No accident occurred and no accident report was filed with company. EE is still working, he has not been told by WC Dr not to work. He is doing physical therapy.
Employee was carrying a bag of rock dust on the north main crosscut 30 when he stepped over a wall and his right foot went into a hole and twisted right knee.
EE was stepping out of lube truck # 172160 and stepped on a rock twisting his ankle.
Employee was packing cement bags on packer when one of the bags burst resulting in the employee getting cement in his eyes.
EE was tramming a golf cart beside of #6 belt line while changing belt rollers. He drove through a low spot on the belt line and he and passenger tried to get lower in seat. EE's SCSR hung on timber and pulled his leg off of ride, leg struck timber and fractured femur.
Employee was working on a monitor in Cathead on Feeder, he was on his knees and had a cathead in his hands getting ready to hook to the power center when he had severe pain in his lower back.
As EE was getting out of a mantrip at x-cut 65 in the left mains, his left foot got stuck in the mud and he strained his left knee as he lifted his leg. EE continued to work until 6/16/2010, at which time he had surgery on his left knee.
Employee was inside lowering chain to another employee when weight of chain became too much for him to hold. When the chain un-rolled and fell to the ground, it caught his right thumb. Small abrasions and a sprain resulted.
EE got off of the mantrip and said his back started hurting.
Employee was removing a shuttle car tire. As the tire fell of the lugs, it struck the employee in the right ankle resulting in a fracture.

Injured claims he was stepping out of the truck and his right pant leg got caught on fire extinguisher. As he started to fall he attempted to catch himself on the truck door, missed and landed on the ground with his left foot between two rocks and his head hit another rock. Injured stopped to clean off headlights.
Roof bolter operator was walking beside his machine to get a drink of water. A piece of material fell above the wing curtain and struck him on top of his head. The incident resulted in a hairline fracture of a cervical vertabrae facet.
The employee was laying in a panline when he began to slide down the pan. He put his leg up to stop himself and struck it on a roller. The employee felt a pop in his left knee. The employee did not begin to lose time until 6/14/2010.
Employee was shoveling mud (barite) up off the floor into the shaker (dewatering screen), which is about 5 foot high and his knee gave out on him. This is a repetition job to keep his area clear of hazards and clean as needed.
Employee had stopped miner to pull up miner cable for next cut. After pulling and lifting cable he felt pain in his lower back while walking across section. Resulting in a strain to the lower back.( Employee worked until 8/4/2010 and then became LT)
Shovel hit haul truck, jarring employee. Initially released to full duty after ER visit. Was working on 06/18/10 and unable to lift chock blocks. Returned to doctor and put off work. Lost time began 06/19/10. Currently attending physical therapy.
Employee was doing ""utility"" work,(shoveling), in the C6 tunnel. While shoveling he felt a ""pull & pain"" in the right side of his abdomen & groin area. EE, went to see a doctor on 05/25/2010 and called off work until 05/31/2010. ee returned to work on 06/01/2010. There were no ""restrictions"" noted about his ""return to work"",.
Operator was access haul truck/Stumbled on access steps to operators cab. Landed on his right wrist, worked for two and one half hours. Wrist began hurting and swelling, went to hospital at 10:00 AM. treated and released. In doctors care to date.
Employee was roof bolting and they advanced the bolter for their next row. The employee was swinging the boom out,when a rock fell out striking him on the head. It knocked him to his knees and the sharp edge of the rock fell on his left calf muscle. The employee was taken off work by doctor started missing work on 06/01/2010.
Employee stated he was bolting and had dropped his boom down against the floor, with pressure on it. He then lowered his canopy and raised the boom off the floor at about the same time. This caused the canopy to come down faster than he thought and it struck him on top the hard hat. He worked until 06/24/2010 when taken off by his doctor.
Employee felt pain while mucking and was having some numbness & tingling on his right side. He came to medic on site, and we transported him down to ER - Doctor put him on restrictions, RX was issued and MRI is set for 5/28/10.
Miners were attempting to tram a continuous miner by installing power wires directly into a receptacle in a power center without using a proper disconnecting device. When they attempted to set and reset the breaker, the breaker arced causing serious burn injuries.
Employee was coming down the stairs from the top of the additive silos (approximately the third set of stairs from the top) when he slipped backwards, grabbing the handrail for support with his right hand, when he felt pain in his right shoulder.
EE stated that he was roofbolting in #6 entry, and was partially under the canopy when he was struck by a piece of rock (2'X 3'X 2"". The rock struck the canopy then rolled off the edge of the canopy. EE had neck pain and tingling in his left hand. EE RTW on hi snext shift,but began missing on 5/25/10.
EE was hauling a load of coal up a grade after a rain shower and truck spun out. EE attempted to stop and put truck in reverse and missed getting it into reverse and truck slid backwards, down the hill and hit the berm and turned over on left side.

Maintenance employee was working on an aggregate discharge chute under a non-operating screening unit, from a step ladder. Weather conditions were damp, light rain. Employee slipped from ladder step while reaching for a part, falling approx. 6-7 ft, landing on a conv. tailpulley guard on his left side, resulting in a mild concussion and 4 broken ribs.
Employee was tightening up a come-a-long and the chain broke. Chain hit employees rt hand, and broke rt middle finger.
Employee stood on scalper bin lid and the lid gave way. Employee fell into bin. He caught himself on the side of bin causing a contusion to the side of his left chest.
EE was cleaning (shoveling) the #9 head drive. EE had a shovel full of gob and twisted to throw gob on the belt line when he felt pain in the lower part of his back.
Changing blow bars in Universal 130/150 Impactor, EE was attempting to load a bar into tractor bucket to transport. Lifted more than he should. He thought he had just pulled a muscle below his waistline on his right side. He chose not to go to the doc. Had an annual checkup on 6/7/10 and it was then that it was discovered he had created a hernia by the lifting.
Employee was stepping off of a ladder and twisted his ankle.
Two employees were relocating a plastic water line in the field. One employee was on the ground directing the other employee in the loader. When the loader moved, the plastic pipe shifted striking the employee on the ground in the chest. He was taken to the local Hospital for observations.
Employee was walking intake escapeway per company safety day. He was tethered to another employee and holding on to lifeline. The lifeline went around a turn and there the rib had rashed out into his path. Employee twisted his left knee on the loose coal.
Employee was shoveling in storage unit casting gob up on belt that was overhead high and strained his shoulder. EMPLOYEE DID NOT START LOSING TIME UNTIL 4/5/11 DUE TO SURGERY.
Employee helping pull a new conveyor belt on to a mobile crushing/screening unit. He was pulling the new belt with a rope on ground level, when he stumbled and fell over loose ground. His right ankle was injured.
EE was in the process of jacking a derailed track flatcar back on the track, the jack bar slipped, causing EE to twist his lower back and left hip.
Right foot slipped off the last rung of ladder. Hurt right knee.
The EE was hanging the miner cable by himself when he felt pain in his right testicle. He did not go to the doctor at that time. EE went to the Dr. on 9/22/10 and was diagnosed with a hernia. Surgery to repair the hernia was performed on 10/27/10.
An examiner reported a loose rock to the section face boss. The face boss went and got a pry bar to pull it down with, when he pulled the rock down it landed on its end and tipped over, striking the boss on his left knee. This happened between the #1 and #2 entries in the last open cross-cut of the 2nd East panel.
While stepping off ladder from a screening plant EE states he stepped onto a rock and twisted his right ankle.
The employee stated that as he was crossing the extendable tubing (baloney skin) attached to the back of the auxillary ventilation fan, he lost his balance and fell backwards onto the baloney skin. The force of the exhausting air caused him to jerk back up into the upright position. He later complained of back pain.
He was carrying his tool bag and felt pain in his lower back. He continued to work at his regular duties until 12/21/10 when he was taken off work for back pain. He has a history of back problems dating back to 3/14/02. This history information and the physician reports are included. Please consider this occupational Illness due to repeated trauma.
Employee was using a handheld electric drill motor, drilling holes in steel plates. Drill bit caught in the steel, while drilling. Drill motor twisted in the hands of the employee and twisted right wrist. Injury was accessed and employee was advised to see a doctor for a sprained wrist.
Helping load rock duster in Mac 8 mantrip. Pulled something in upper leg.
Employee stated he was installing an 8 ft. bolt into the roof. When he went to bend it back straight his hand slipped and the bolt twisted straining his shoulder. Employee worked the next 2 shifts and started taking off on 5/23/2010. We was unaware he was missing work until 6/02/2010 and why he was missing. We did not receive a Dr's note until 6/03/10.
Hose used to wash scale came apart and clamp cut employee's left hand, requiring medical treatment.
Unloading tool box, caught and mashed hand between acetylene tank and tool box.
During replacement of a 22.5 degree fitting on the 20 Matrix line, pipe was being moved by rubber tire. When the pipe was lowered, the pipe sprang off the forks towards the injured employee, pining both of his legs (shin) under the pipe.
Employee was building stopping and felt pain in his right wrist.
Mechanic was working on a Bell articulated truck engine, removing misc exhaust hoses. He was leaning on a wrench and slipped, causing his forearm to come in contact with something that resulted in a laceration. The right forearm was affected.
Employee working on mezzanine level between floors. Employee crawled under pipes to reach work area. Surface was metal grating scattered with loose cement fragments. While crawling knee injury occurred. Employee was able to work the remainder of the day, however the knee began to swell. Employee went to physician next morning where it was confirmed to be a sprain with swelling.
Employee was adjusting make-up water to cooling tower when a hose came out of the top of the tank and sprayed employee with hot process water. Employee received burns on right side of his body.
Employee was putting a return roller up over his head, on conveyor #1, he slipped, and stepped backwards onto a rock, rolling his left ankle.
Injured was moving a continuous miner cable when he felt pain in his lower back.
EE was preparing to work on a shaker when he slipped and fell from it falling about 8 feet onto the concrete floor. The fall broke a bone in his elbow. Surgery was required. Healing time is unknown, but it will be several weeks.
Employee was drilling a hole for a roof bolt installation when a section of roof around the hole loosened and came down on the drill steel, hitting the drill pot & then hitting employee on the knee. Employee sustained a contusion to the knee. Doctor restricted employee from working on 5/21/2010.
While mucking rock, employee injured his back.
EE stated that he stepped of haul truck ladder at end of shift and aggravated a pre-existing back injury. EE stated he twisted back, possibly from a loose stone or small hole. EE did not feel any pain at that time, but woke the next day with back pain. EE's attending Dr. gave him 2 days off work, and returned to work with no restrictions after his scheduled week of vacation.
Employee stated that he tripped over a water hose & fell. An MRI showed moderate degenerative changes & ACL partial tears. This will be the third surgery on his knee due to degenerative changes & ACL issues. Employee's first lost work day did not occur until 9/8/2010 due to surgery.
Employee injured left elbow when he slipped and hit elbow against handrail while shoveling sludge.
WHILE USING A SLEDGE HAMMER; EMPLOYEE WAS REMOVING SPROCKET SEGMENTS ON AN UNDERCARRIAGE. A SECTION / PIECE OF THE SEGMENTS BROKE OFF AND PENETRATED EMPLOYEE'S LEG. (RIGHT) SHARD WENT ALL THE WAY THROUGH HIS LEG.
Injured was removing a staple lock from a water hose. When the staple lock came loose, injured fell striking his ribs. This resulted in a bruise.
Stepping into the pickup truck at job site and left leg popped out, knee and leg swelling.
The employee was using a hammer to dislodge a rock from a grate. A rock fragment broke off and stuck into his hand between his index and middle finger.
Employee twisted her knee while walking in a wet area. **DID NOT START LOSING TIME UNTIL 6/10/2010**
While pulling on belt, the employee felt pain in his back
Employee was preparing to place platform in place for stacker. Employee stepped close to edge of one block, which was four feet from concrete floor. A 10 inch by 22 inch piece of concrete block broke off, causing employee to fall to the ground, injuring his left hand and left knee.
Employee had hooked up a chain between a battery scoop and a shuttle car. The scoop bucket was on the ground at the time. He asked the scoop operator to back the scoop up. He did not realize that his foot was under the side of the scoop bucket. When the scoop backed up it drug across his foot
Employee was tramming lo-trac and hit a rock in the roadway jarring his back and leg. EMPLOYEE ONLY LOST TIME FOR ONE DAY ON 6/15/2010 DUE TO PROCEDURE REQUIRING HIM TO BE OFF FOR 24 HOURS.
Employee stated that as the manbus he was riding went through the truck doors on way outside, the doors shut, causing the door to strike the manbus, bouncing his foot out of the ride. The doors then struck his foot as the bus struck the doors the second time.
He was replacing main pumps back in the boxes to take back to the shop, when he slipped and fell from his truck bed and struck the bumper in his back. He was in pain, so had to quit working.
Employee was cleaning material from the top of a Mefcor spill switch when he received an electrical burn.
Employee was loading shear head jack when his foot slipped and he fell out of motor. Employee started missing 05-24-10.

It was raining outside and employee was running into office and slipped on porch
While lifting miner cable and water lines to hang, felt pain in left shoulder, it gave out.
EE knelt down to check oil in 4 wheeler when he felt a pop in his left knee. EE stood upright then knelt down again and felt another pop. He was treated and released at Raleigh General with a follow-up on May 21, 2010 with an orthopedic. He determined that the EE had a torn meniscus and performed a procedure to insert two staples.
Roof Bolter Operator bending roof bolt and felt back ""pop"".
Employee was using a hammer to reposition a return roller to train the belt. He swung the hammer and missed the roller and his hand went into the roller, causing a fracture and lacerations.
Shift Supervisor opened port of dust collector airslide to check for blockage, when material surged, blowing dust in face.
Water line had been turned off. When turned on, manifold blew off and hit employee on right leg.
Employee was bolting the back, he had drilled one hole & started to drill one more hole when a rock fell out of the back, pushing the jackleg back. Employee was running away & fell to the ground and the jackleg ended up landing on top of him. We transported him to ER, and the diagnosis was 4 fractured ribs and lung contusion.
When hooking and un-hooking shields, chain caught on pontoons and EE strained his groin pulling on it. He was taken off work on 5/27/10 for a hernia.
EE was shoveling bentonite and strained lower back. Attempts to run 627G scraper on the 17th and 18th met with severe pain. EE went to doctor on the 18th and was given a no work slip until 05/24/2010, at which time he returned to full capacity.
Water line had been turned off. When turned on, manifold blew off and struck employee on right arm and leg.
EE twisted his knee getting out of the mantrip in the motor barn outside the mine. He saw a doctor and was told he needed to have his knee repaired but due to blood pressure problems they would not schedule surgery until 7/26/10. He continued to work his normal shifts until that date. EE began missing work on 7/26/10 when he had the surgery.
He was washing down the slope belt facing inby, when he stepped on loose material causing him to fall and twist his left ankle. He continued to work at his regular duties but continued to have pain. He was removed from work on June 14, 2010 after an MRI indicated a nondisplaced talar fracture.
After shanking the steel on the jumbo, employee rattled it loose, grabbed steel to un-screw it and the collar started to un-screw also. He grabbed the collar to hold it & it was hot from the rattling steel, and it burnt the palm of his left hand, resulting in 2nd degree burn.
EE slipped and fell in a mud hole. This strained his left knee. It is unknown at this point in time if surgery is needed.
When getting into service truck, EE reached for the grab rail to pull himself into the truck. When he did he felt a sharp tear like pain in his right shoulder. EE did not mention this incident until a few days later when he sought medical attention.
The man was lifting material while setting a belt tail and developed pain in his left lower abdomen.
Employee was drilling top to install roof bolts a piece of draw rock fell and hit drill canopy, fell off canopy and hit coal rib then hit employee in lower back causing a bruise
EE was walking around the section power center when he tripped over another EE's feet. EE fell to the ground causing pain to the left leg. EE continued to work the remainder of the shift. Later in the day EE went in for medical attention. He has a fractured tibula to the left leg.
EE was trying to push up a bent roof bolt. The wrench he was holding slipped, causing drill to mash his right middle finger.
Rolling large rock off haul road and felt pop in right shoulder.
Employee was struck in the mouth while trying to unclog a rock dust hose.EE needed 8 stitches to bottom lip.
Was changing grease fitting on track hoe boom, ee backed fuel truck up to stand on caught another ee between rear of fuel truck and the track hoe boom. Mashed and tore right shoulder and arm pit, few cuts and bruises on right arm, chest and back. Ambulance was called ee was transported to Erlinger Hospital.
On the above date, EE was working on the company 610 scoop checking permissibility. He had finished adjusting the brakes and was putting the shield back on. As he pulled the shield, he felt a sharp pain in his lower abdomen. He went to the doctor on Monday and was diagnosed with a hernia.
The employee was putting a hitch in a trailer when he felt a pain in his neck and shoulder blade. The employee continued to work until 6/8/2010 when he started missing work.
The injured was installing a roof bolt, he was spinning the the drill chuck and bolt counter clockwise so that the bolt head would seat down into the drill chuck prior to pushing the bolt up. He was holding the bolt as it spun; his glove was snagged by the bolt which jerked his hand/thumb around the bolt.
EE stated he grabbed a handful of bolts and twisted around to put them on the tray. While twisting he says he hurt his back. *EE did not report this injury until 5/17/10 and has no witnesses. Suspect injury claim is invalid. Also missed workday before. Found no one to corraborate.
Employee was installing a motor on the batch plant mixer when his left thumb got caught behind the motor. Thumb was de-gloved from knuckle to the tip. He was taken to NNRH where the tip of the thumb was reattached.
Injured was tightening a bolt on the axle of a 9 ton mantrip when he felt pain in his RT shoulder. Having an MRI the injured was found to have a rotator cuff tear and a ruptured bicep tendon of the RT arm. Injured work until surgery on 11/3/10. Injury is being reported as an occupational illness due to repeated trauma over 37 years of working in coal mines
EE was moving a piece of plywood, approx 25 pounds, he sat the plywood down and stated that he had received an electrical shock. The area was looked over and it was found that there was no cords in the area, he stated that he wanted it recorded that he thought he had received a shock.
Hammer slid off wall and pulled on right shoulder.
EE stated: He was shoveling in the area near the 3B header/3A tailpiece. He attempted to use his shovel to pick up 1/2 of a kettlebottom that he estimated at a weight of 10 lbs. He said that the floor area was wet and he slipped and fell onto one knee. This caused pain in his lower back on the right side. He stated that he picked the rock up by hand afterwards.
Employee was walking down the #3 entry, slipped on a loose rock and twisted his rt. knee. Started missing work 8-6-2010.
EE was cleaning raw coal crusher using a walk behind BobCat. EE picked up a 5 gal. bucket of mud and strted backing up a steep grade. The BobCat moved forward the EE slipped and fell. his left foot ended up under the BobCat.
Employee was repairing a battery of a Loader Caterpillar 980C, he turned his back to look for a grinder when the battery exploded. As a result of the battery explosion the battery acid came in contact with employee's left eye.
EE was walking on the catwalk of highwall miner, when the front seal blew out on the fluid coupling below, causing flash burns to exposed skin of arms and face. Resulting in first and second degree burns.
Employee stepped out of the equipment (Gradall Scaler) he was doing a pre-shift on, missed the step and jumped to the ground.
Member was replacing the cutting edge on a blade for a bulldozer using a 1/2"" impact gun. As he was doing this he stumbled striking his back on the tracks of the dozer. Member brought in documentation from his doctor on 5/27/2010
EE used an 8lbs hammer and struck a ratchet to break a volt loose. When he did he felt pain in his right shoulder. EE worked until 8/27/10 and had surgery for a torn rotator cuff. It was initially diagnosis as a shoulder sprain. I am classifying this as an occupational illness due to the report provided by the treating physician. See attached.
Employee reported a low back strain, was evaluated by a Physician on 5-17-2010 and taken off of work resulting in Lost Time. This personal injury has been determined to be attributed to an event dated 12-12-96. *On 5/27/2010, Medical was obtained indicating the above.
A small piece of roof rock fell from the top and struck an employee on the forearm, causing an abrasion.
A piece of mine roof came out between the ATRS and the roof bolter canopy. While trying to remove this rock, a piece of it hit the employee on the back and pushed him against the bolter. He was examined and released that day but has begun to miss work starting June 11, 2010 due to this incident.
EE was trying to open back door on skid-steer loader. He used a file to pry it open. The file broke and cut his left arm.
Employee was walking on Section and his upper back and lower neck impacted a roof bolt plate, causing small contusion to skin. Employee went to Hospital after work and the Laceration was ""Glued"" -- no stitches were made by the treating physician. The employee missed one shift of work and returned to his regular job duties.
EE stated stated while lifting a part from the floor, he injured his back.
Unloading slabs of stone from saw by hand to a pallet and caught his fingers between two slabs. Pinched tips of 3rd and 4th finger of left hand. Mangled tip of 3rd finger. No broken bones.
EE was repairing belt structure at No. 7 belt take-up when a fire suppression spray broke and sprayed water in his face and as he was trying to move away, he tripped and fell striking his left knee on the mine floor.
Section foreman was bent down in the #4 entry, approx 4' inby the outby corner, next to the right rib. Scoop operator was backing into #4 to clean entry in front of feeder. Scoop operator did not see section foreman because the foreman was bent down picking up a notepad. Battery tray end of scoop struck foreman and fractured left femur, pelvis and ribs.
While attempting to install a ground wire to a ground buss bar in a 480 volt breaker panel,the wrench he was using came into contact with an energized breaker buss bar. This caused an arc flash causing serious burns to the EE.
The employee was attempting to get out of his truck and stepped down to the step on the fuel tank. His boots being muddy his foot slipped and his tail bone hit the fuel tank. He didn't go to the doctor for a couple of weeks till his neck started hurting him. He has not missed any work from 5/12/10 to 12/6/10. The doctor wants him off till after he recovers from the surgery.

Injured Employee was struck by a rock measuring 20""l X10""W X 3"" thick causing a contusion to the left side of his face.
Cut finger, required 5 stitches, when attempting to remove guard from #10 conveyor low speed coupling. As she was lifting the guard, her finger was pinched between the guard and motor, resulting in the laceration.
Employee was standing on the bed of the drill repairing a feed chain when he lost his balance and fell onto the axle of the front tire, injuring his left rib cage area.
The man was walking and slipped on wet bottom, causing him to twist his knee. The man continued to work regular duty until 5-18-10.
Operator slipped on top of excavator while refueling and injured shoulder. Operator worked next few days then went for an appointment and local provider. Local provider placed employee on rehab. Surgery was required 2 months later.
Employee was moving a portable generator and hurt his back.
EE worked all day without any complaint. EE stated that he had back pain when pulling a 20' screed board across concrete (strike off board).
Party lifted a sprocket segment which weights about 70lbs out of a box. He felt a pain in his lower back. Missed 1 day of work.
Worker was loosening a bolt and wrench slipped resulting in worker catching himself during fall and spraining wrist.
Rocks bridged up in the hopper and spilled over the edge. When EE went to clear the jam - he rolled his ankle on the spilt rocks while walking to the hopper.
Came out of return, turned belt off. crossed belt. Stepped off belt and twisted right knee.
Employee stated that as he tossed a bag of trash off of the supply car, he felt pain in his right shoulder.
Employee reported injury on 5/11/10 that he had sprained his ankle getting out of operator compartment of lube truck. He continued working his regular shift. He There were no major complaints up through mid-July. He officially began missing work on 7/23/10 when he was scheduled for surgery.
Employee was removing a block wall that was used to store mud behind it in order to be dried and later removed from the mine via Scoop. As he started to remove blocks some mud came loose thus pushing employee's leg against the blade of the Scoop.
Employee was filling water tank with water out of a pump hose and tripped over the hose and fell about 4 ft from back of pickup truck to ground hitting shoulder area.
Climbing ladder to work on discharge. Foot slipped through second rung on ladder, twisting back. Did not fall to ground.
Cause of injury is unknown. Employee alledges back muscle spasms. Employee alledges he was changing screens.
While sitting on a John Deere Gator stopped another John Deere Gator contacted the rear of the stopped vehicle. The employee in the stopped vehicle contiued the shift and reported the incident at 7:12 p.m. On 5/13/2010 he went to E.R. where he was treated and released.
EE was cleaning the area around the tail of Sugar Camp #2 Belt. EE was lifting a railroad tie onto the back of the belt truck and felt pain in his lower back and down his right leg. EE completed his shift and did seek medical treatment on the following day, however did work next shift. EE was diagnosed w/pulled muscles and part torn tendon by dr on 5/24/10, placed off work.
Employee was putting new hydraulic pipe under the body of the Hitachi shovel. He was standing on a short ladder and slipped. He grabbed a hydraulic pipe to keep from falling and hyperextended his left shouler.
Using a bolt plate to cut strap and it slipped and cut hand between thumb and pointer.
Employee was pulling klinkers and experienced pain in lower back and down both legs
Employee was working in the MCC room of the plant dryer checking out electrical contacts. While testing the contacts, the contacts caused a flash while he was standing by the control box. He did not think he had any problems until he woke up the next morning and he had pain in both eyes. He was diagnosed with corneal burns and was given eye drops to releive the condition.
Employee was lifting miner cable to put a sling on and felt pain in right shoulder.
Employee was roof bolting and as he reached back with his left hand to the roof bolt tray he put his right hand on the drill pod and a rock fell from the top hitting his right hand.
Operator was climbing down a ladder on the side of his loader when he felt a strong pull in his leg. Pain continued and operator was transported to the hospital for an examination and Dr. review.
Uncoupling a five car trip from a 10 ton motor while standing between motor and supply car. Cars didn't uncouple and the motor was pulled on top of his foot, fracturing it.

On May 7th, 2010, the employee was both pulling and lifting on a hose to attach to the water truck when he began experiencing upper back pain. Then, on 5/28/2010, the employee began missing work as a result of this accident.
Employee stated that he stepped up on the bumper of the bolt machine to get a box, when he stepped down from the bumper he twisted left knee.
The injured miner was changing oil in primary crusher feeder. To access the feeder he was using a six foot step ladder. He fell from the ladder an undetermined height and struck his head/body on the ground causing his injuries.
Employee was getting into the shift change van. While entering the van on the side door, the employee slipped. It was dark and wet outside. His shin hit the metal edge of the van, causing an open wound.
EE was holding steel aligning it with jackleg and jackleg jumped smashing his hand into the roof.
Employee was pulling the continuous miner waterline while walking backward and tripped on a rock and fell and landed on his right hand and injured his right shoulder.
EE was bolting on the left side of the Joy miner and finished the bolting cycle. EE walked to the rear of the miner and with no warning the rib (22' long & 13"" thick) rolled out striking him in the head area and pinned him against the miner. EE suffered a broken collar bone & a bruised lung.
Employee was carrying a tray of block to West Mains when the employee went to set down the tray of block. The employee stated the hydraulics stuck and the tray hit the ground and throw the employee into the top of the cab. (The loader was checked and everthing worked properly).
Employee was stacking bags on a pallet, when he felt a muscle cramp in his calf, he then felt and heard a pop in his calf muscle of his left leg. The employee immediately reported the injury to his supervisor.
Roadway/coal rib material had been pushed into the face during the clean-up cycle. The CM operator started loading the material with the CM and picked up a piece of metal, located in the material, through the conveyor chain that flew out of the chain and hit him in the head.
While recovering Kennedy stoppings pulled several panels down when material held by the panels fell striking the outside of his right ankle. Transported by private vehicle to MSHA Springfield where X-rays revealed a fracture to the right heel. He has not returned to work.
EE was on the quarry site for demolition of Liebherr R984 front shovel. EE pulled cylinder pin out of the boom and the cylinder fell on his foot.
Door slamed shut when backing up, ground gave away.
Employee reported soreness in shoulder after swinging 16 lb sledge hammer. Employee states shoulder has been hurting for some time (a year or more) but is hurting worse now.
The employee was cleaning the crusher with the water hose, he slipped sideways.
Rotating pot & valve on underflow of pump. Tight & confined space. Had to reach around valves to get to bolts to tighten them. While he was doing this he felt his shoulder getting very sore. The 7000-1 is being filed now because the Safety Dept. became aware on 7/21 that he was unable to do his regular job tasks.
EE was unloading a roof bolting machine canopy jack from the maintenance truck with a chain and hoist at the surface shop. As the jack was being raised, and EE placed his hand on the jack to guide it, the jack slid through the chain and mashed his right index finger between the jack flange and the hoist hook.
Employee was moving used oil drums on a flat bed trailer. Employee pulled on drum he thought was full but was actually empty. The lack of weight caused employee to lose his balance. Employee fell backwards striking his shoulder against forklift and then the ground. Employee received a contusion to the shoulder and was seen by a physician on 6-1-2010.
Was putting up a pogo stick to hold down a ventilation curtian when he felt pain in his left hip and leg.
Employee stepped from bolter to footwall onto mud and uneven ground and rolled his foot over.
Employee & co-workers replacing belt drive motor. The motor was blocked and lifted by come-a-long onto the bed plate. Employee using a rock bar to assist in positioning the motor onto the bed plate. During positioning motor, supporting eye-bolt stripped out of top motor. Motor fell 10"" striking employees lower left leg causing a simple fracture.
Stepped out of the bulldozer to urinate, the sidewall caved in, burying EE waist deep in gravel, pinning left leg against the track. Hit head on the ripper arm of the dozer, split head open.
EE was cutting off screws on old sheeting when sheeting started to fall causing the grinder to be knocked into his left forearm causing a laceration. EE needed 6 stitches.
The employee was adding oil to a tow boat when he stood up and struck his head on a steel beam. The employee felt a sharp pain in his neck.
Employee was pulling a water hose when the hose gave way. Employee hit his right elbow against a handrail.
On 5/5/10, the employee was staarting the shift and was climbing the mounting ladder of a 835 Volvo articulated truck. The member's foot slipped and he grabbed the handrail with a free hand & in doing so straind his lower back. He missed no days. In September the member again saw a Dr and after diagnostics were completed was placed off work on Sept 27, 2010.
Not reported until 5/7/10-Miner was cleaning under crusher platform when he stepped on a 4"" rock and twisted his right knee while pulling water hose. Miner's first day of lost work was 6/23/10 for surgery.
Member was pulling rib and was hit when the rib rolled and pushed ee into the roof bolter.
The loader the individual was running caught fire. He then noticed flames and jumped from top of loader. When he struck ground, he thought he hurt his knee. The ankle was sprained but broke his heel.
Employee was carrying a barrell of grease down steps when his foot slipped on the gravels.
Employee and another assistant were using a slate bar to carry a dolly of the push/pull unit from the belt entry to the intake entry. While walking backwards, the employee stepped onto a bundle of wedges causing him to stumble. The weight of the dolly shifted on the slate bar towards the employee injuring the employee's back.
Employee was removing bolts from head pulley on a belt conveyor when the head pulley fell knocking employee down.
Employee was making a belt splice, hammering nails into splice when he felt pain in his right shoulder. He said it was probably where he had a steel plate in it from a previous accident. Didn't report it at first because I thought it a reoccurence of an old injury. Then Dr. said it came loose due to work he was performing.
Employee was fueling a D11N dozer when he slipped and fell to the ground.

EE was standing beside the mine track at the portal and the rockduster wrecked off the track and caught his left leg. Fractured leg. Started missing 5-5-2010 and RTW 8-23-10 (75 days lost). EE started missing again due to complications on 11-1-2010.
Employee was walking and twisted right foot.
One employee was moving belt over and the fork fell off and struck another employee on the right foot.
Employee engaged main breaker in power room resulting in fire. The force of explosion, fall or combination the employee hit head and shoulder against block wall and door landing on concrete floor.
Employee was changing the conveyor belt #2 of the cement Finish Mill #2; while in the process, employee picked up an end of the chain and launched towards the other side of the belt experiencing an intense pain on his left arm shoulder.
EE was attempting to change the position of the flopgate at the head of the #2 raw coal belt. The flopgate had become wedged against a small bolt head inside of the chute.While using a prybar to dislodge the gate the bar slipped out of the EE's hands. He grabbed the bar as it fell and wrenched his shoulder. EE started missing work on 5/11/10.
Injured was carrying oxygen tank and stepped in hole, twisting lower back.
Employee was tossing a rope across a small dewatering hole to another worker.
The accident occurred while installing beams in the slope area. EE was helping to install the beams when the spacer bar rolled into the beam injuring his thumb. His thumb was caught between the spacer bar and the beam resulting in the need to seek medical treatment for a laceration and other possible injuries to his thumb.
Employee was lifting a drill guide assembly from the back of a piece of equipment when he felt a pain in the right lower abdoman.
While performing pre start-up insp. on machine, operator walked around catwalk on excavator (245 Cat), catwalk broke and operator fell 5' to the ground, landing on his back and hitting his head. Operator was wearing hard hat.
Two men were carrying an 8"" x 20' plastic pipe. The employee helping to carry, slipped, causing other employee to twist his back.
Belt rubbing stand. Took belt rail and tried to move structure over.

Walking with cap board, tripped and fell, smashed arm.
Employee was hanging roof bolter cable when the cable fell down striking him on the right shoulder.
EE was changing a jig seeve on the prep plant 4th floor. He was carrying the jig seeve down 4 steps with another ee when the seeve twisted to the right causing ee to receive a cut between the thumb and index finger on the right hand.
The employee was holding a steel strap in place while installing a roof bolt. When he applied pressure to the roof strap he pinched his finger between the strap and the mine roof.
Walking from Section Power Center to the Roof Bolting Machine slipped and fell twisting left leg at knee
During the beginning of May, employee injured his knee while jumping off of a vehicle to the ground. At the time he did not feel it was necessary to see a physician. Towards the end of July, he expressed concern that his knee was bothering him. He was sent by Cannon Resources on August 11th to Denver to see his primary care physician as requested.
Employee was standing on a curb outside of bathhouse near parking lot and another employee accidently stepped on his foot. Employee lost his balance and fell from curb fraturing his right ankle.
While driving a truck which was being towed (mud), ee was struck in the forehead by a chain when it failed. EE recieved 3 cuts to his forehead requiring stitches.
Employee was assisting another employee to remove a nipple from a 1 inch ball valve. He was using a pipe wrench and as he pulled the pipe wrench toward his body he felt two pops in his shoulder followed by pain. Diagnosed as a dislocated shoulder. Work restrictions applied 5/31/2010.
Employee was trying to move the track on a dozer when the bar he was using slipped, causing him to start to fall. He attempted to catch himself with his left arm. Restricted duty injury as of 6/3/10
Employee was taking a wheelbarrow full of light material contents to the dumpster. When he dumped the material he felt a sharp pain in his lower left back. Resulting in left lumbar strain.
Employee was using a battering ram to remove lock nut from a crusher mantle. He felt sudden pain in left shoulder.
EE had just finished lunch and was walking back to the ball mill when he said somehow he twisted his ankle. I took EE to the clinic in Lordsburg, NM. EE had his right ankle X-Rayed,no broken bones. The PA-C told EE not to go back to work until Tuesday, June 1, on light duty.
Miner was taking a light off light pole, unhooked the brace the light swung around and hit miner in right ear and head.
Removing guard under 60"" belt. EE was laying under the guard holding it with one hand. Could not hold it and dropped the guard on thigh.
Traveling inby on 3rd NW Travelway @ xc-12 driver's foot became wedged between gas and brake pedal, pinching employee's left ankle between mantrip and rib.
EE states a co-worker was pulling a wheel barrow up the stairs when the wheel barrow slipped out of the co-workers hand the EE fell on his knee getting out of the way. Medical treatment was given and the EE missed one day and RTW to full duty on 5/31/10.
While operating a continuous miner and cutting the slab side of a straight cut a rock slid off the miner and struck his left foot leaving a laceration. Transported by ambulance to Memorial Medical ER Springfield where X-rays also revealed a fracture to the foot.
IE had moved to the next hole in the pattern, as he leveled the drill he mistakenly extended one leveling jack. This allowed the drill to lean to one side and eventually fall to the dust collector side of the machine. In the incident the IE struck his right shoulder inside the cab. He had rotator cuff repair on 9/9/2010
Employee was walking down the long wall face on the toes of the shields when a rock fell between the shields striking the employee on the thigh. He finished his shift and the following shift. The employee noticed his thigh starting to swell and went to the hospital where the DR. found the bruised thigh needed the pressure relieved. The wound needed to be stitched to close.
Employee was traveling down a stairway when he stumbled on the bottom step. He twisted his Left Knee as he caught his balance.
The employee was assisting a co-worker to position a removable section of floor grating back into position. The floor grating was setting in position to cover the floor opening, but injured employee place his foot on the corner of the grating to further position it when it tipped down. The employee lost his balance and fell approximately 9 1/2 feet to the lower level.
While dusting # 10 entry of the 2nd SW with the # 1 battery scoop and slinger duster, the IE tripped over the front of the bucket. When he fell, he fell forward, and in the process of trying to catch himself, he strained his right shoulder. He continued to work with the pain, until he had surgery to repair a tear to his labrum on Februuary 21, 2011.
Employee was roof bolting in section 003, they had just installed the next to last row of bolts and they moved up to install the last row. The injured employee reach in to the controls to swing the boom of the bolter, when a rock fell from the top and struck employee driving him under the boom of the bolter. His right leg was broken in two places below the knee.
EE was loading metal roof straps from the scoop car to the continuous miner. EE raised the straps to allow them to be pushed into the strap rack when he felt pain and a pop in the left shoulder.
Employee walked into an airslide, hit his head which caused him to jam his neck. He has been under medical observation only, but on 6-11-2010 the doctor felt he needed physical therapy, which makes this accident reportable on this date 6-11-2010.
Injury was using a hammer wrench. A nut came loose and he smashed his finger between the wrench and a speed reducer.
Miners were attempting to tram a continuous miner by installing power wires directly into a receptacle in a power center without using a proper disconnecting device. When they attempted to set and reset the breaker, the breaker arced causing serious burn injuries.
Injured employee got his left hand caught between the roof bolter canopy and the mine roof.
He was using a chain, pulling a shaft with a come-a-long. The chain slipped causing the pipe to hit his left leg. He went to Mercy Medical in ID. The x-rays showed a broken fibula.
Air diverter head was being removed from the 7"" surface casing, using a tool winch. The head was stuck on the casing. EE reached in to straighten the head when it suddenly came loose crushing the first three fingers on EE's left hand between the head and the drill table. First two fingers were broken all 3 fingers were lacerated.
Miner was helping install a crusher wheel with an overhead trolley system. When the trolley got jammed the miner tried to free it, his left little finger got caught between the trolley wheel and the I beam causing it to be cut.
Twisted left ankle when he stepped on a rock.
He was kneeling on his left knee replacing a belt on a transfer conveyor. When he stood up he felt pain on the inside of his left knee.
The man was loading belt structure onto a skid. The man had gone to the doctor and was told he had tendonitis. The man's shoulder had not improved since May 2010. The man was scheduled for surgery on 2/4/2011.
He was lying on the belt under the Jaw crusher to tighten the toggle springs. He set the tools down to roll over to the other side. He noticed a pain in his back as he rolled over. He finished the shift. He reported to work at our asphalt plant on 5-21, 5-22, and 5-24. On 5-25-10 He said that he was experiencing back pain and was unable to perform his regular work.
Bin truck driver twisted left knee at the end of his shift. Saw Dr. on 5-20-10 and was released back to duty driving truck. 5-27-10 follow up with Dr., MRI referral and restricted duty thru 6-1-10. 6-1-10 follow up with Dr., referral to orthopedic surgeon and relased back to duty driving truck. Will update 7000-1 following appointment with orthopedic surgeion on 6-21-10.
EE was trouble shooting a breaker panel, as he moved away from the panel there was an electrical arc flash resulting in burns to his hands and face.
Employee operating the splitter. He was moving it and the splitter hit an uneven area and dropped and rocked and shook employee around in his seat.
While Traming a scoop, the scoop hit a hole in the roadway and threw the scoop operator into the canopy of the scoop.
On 01/21/2010 emp was installing Finish Mill liners using a com-a-long and guiding liner into position / liner moved suddenly emp reached out to stop movement and felt strain in Lt. shoulder. Emp was examined /possible tear in shoulder emp. remained at job and attended PT. Due to continued discomfort emp had surgery on 05/18/2010
EE states that he was walking, slipped on mud, fell on left shoulder. Did not report until the following day. On Tuesday, May 25, 2010, EE did not report to work as scheduled. He lost work days from May 25, 2010 to May 27, 2010. EE has returned to work with restrictions. Injury was originally classified as non-occupational by the EE.
Standing on ventilation tube while fixing ventilation tubes and tube he was standing on rolled, he fell and struck the ground with his left shoulder. He began losing time his next regularly scheduled shift.
While installing roof bolts, the employee heard the roof crack, he tried to retreat but was struck by a rock that fell from between the bolts and the rib.
EE was pushing take-up track on top of other take-up track when he slipped on a longwall hose and twisted his right knee.
Three employees were turning over a piece of plate steel. A miscommunication caused one employee to get his right middle finger pinched between the plate steel and the burning table.
Employee touched his left knee to the side of the finish mill causing a burn. Visited Dr. with restricted duty and prescription. On 5/18/10 employee was admitted into hospital for infection.
EE was operating a wheel loader and was filling in ruts of an infrequently traveled roadway with material. Employee went to family doctor whom indicated the back pain he was having was ""possibly"" work related. Employee is having an MRI (WC) to determine damage/severity. He is on light duty, and prescribed meds for night time use only.
The alledged injury is stated to be from repetitive trauma from operating a rock truck. No accident or incident report was filed until 5-13-10 with the company. The medical treatment seems to be limited to diagnostic x-rays and Chiropractic Adjustments.
The employee was performing trouble shooting on the electrical ground monitoring system of a 480 volt receptacle located on a 500 KVA transformer. An arc flash occurred and the employee sustained 2nd and 3rd degree burns to his hands, face and arms.
EE walked to backhoe , the water truck had just gone by making his boots muddy. He slipped while getting into the backhoe. He twisted his arm while holding onto the handrail. He did not report incident until 5/17/10, when bicep was severely discolored. He was taken to the company doctor and diagnosed as sprained right shoulder and torn right rotator cuff.
Employee was using a down gun to drill holes in the bottom, when he raised the drill gun out of the hole he felt a sharp pain in abdomen.
A piece of rib approx 3"" thick X 8-10"" wide X 8' long struck him on the hard hat. It pushed his hard hat down on to him, bruising his forehead. It also bruised the back of his left knee. He sustained strain to his neck.
Employee was operating a small battery-powered transportation vehicle when it ran over a rut or obstruction in the roadway causing the steering wheel to slip from his grip and spin. The steering knob struck him on the hand.
EE released the locking mechanism on the tag line for the cable crane bucket, and it uncoiled and it violently struck him in the left shoulder / shoulder blade a couple of revolutions. He was wearing all his equipment and a life jacket took a majority of the energy. He was sent to doctor for examination and treatment for some bruises/abrasions.
While in the process of hanging a mirror in the mop/wash room of the mine shop the glass broke sticking a piece of it in his right arm.
Slipped and fell, injuring his left wrist
Employee was working on left front tire rotation on Haul Truck 420. While installing the lock ring in place on the rim, the ring popped off & fell to the ground. EE had his hand on the lock ring near the opening. When it popped off, the lock ring closed on his left middle finger causing a fracture to the middle finger. He was placed on restricted duty.
Supplyman was loading a motor with the aid of a chain hoist & slate bar. The clevis broke on the hoist & he fell backwards. He said he strained his back. He stated he was ok & continued working. It was learned on 6/10/2010 that he had seen a doctor & was working in a different job capacity. He was taken off work on 6/10 until the doctor releases him for his regular job.
Climbing out of chute and lost footing on side of chute. Came down on rock, turned knee and required surgery two months later.
EE states he was injured when the sledge hammer he was using deflected off a bull pin and hit him on the right shin. No medical treatment was needed at time of injury. EE developed an infection and sought med treatment on 5/14/10 where he was placed on modified duty and lost one day. RTW 5/25/10.
Injured was in the process of moving a barrel of chain. He stated he was rolling it to the fork lift and heard a popping sound in his right shoulder. He went to the Dr. He did not receive a diagnosis & was released to return to work. On 5/13/10 injured went for a follow up & was placed on restricted duty & referred to therapy. Injured was taken off work 8-17-10 for surgery.
A rock fell off of the face and hit EE in the foot while he was loading the round.
The employee was setting rails in our M4 Main. As a rail was being placed, it pushed a rail jack over, striking the employee in the arm.
EE was pushing a wheel barrel and was too close to an Ibeam. He bumped into it and hit his hand.
While working on a stone splitter employee was pushing a large rock down the rollers on the splitter and felt a pull under left shoulder blade
Reaching to move hydraulic hose to check for a leak. Cut finger tip on frayed wire from hydraulic hose.
Following the cleaning of a light vehicle, dry polymer was spilled onto the floor of a treatment plant work area. As cleaning activities continued the polymer became wet. The operator stepped and slipped on the polymer and felt a pull in his back.
Employee was working on a water truck and was using the tailgate to access the rear axle box. Employee was climbing from the ground into the back of their pickup when the employee felt their right knee pop causing pain to the employee. Employee had surgery on 8/20/10 to repair a torn meniscus and returned to work on 8/23/10 with work restrictions.
Injured employee (Truck Driver) got debris in left eye. EE doesn't know how it happened. Operated truck with windows and doors closed for entire shift before injury. EE taken to the local ER for treatment.Modified duty for remainder of the shift and next scheduled shift.EE released to full duty. One day of modified work. No lost work days
On 5/28/10 EE was dismounting a piece of equipment as he stepped down he sprained his ankle. He was transported to Dr. and was placed on restricted duty until 6/1/10. At which he was to return for follow up. (Holiday weekend) EE returned to Dr. this office did not receive Dr. reports until 6/18/10 to determine status of injured.
Struck by vehicle driven by co-worker. Injured worker was washing/cleaning up his work area at time of accident. Restricted Duty began 7/28/2010
Standing on machine and slipped. Reached out and hung on to air filter housing and pulled tendon in wrist.
EE was usging a dewalt 3/8 drill to drill hole in steel. While using 5/8 bit the drill got stuck and twisted, catching employees right hand and breaking a small bone in hand.
Employee stepped off edge of trailer onto rock on ground twisting right knee. Treatment delayed until 6/1.
EE was helping drive pin out of rock breaker and the hammer glanced off of punch, striking him in the hand, causing a bruise. EE was seen by doctor and released back to his work. EE has since returned to doctor because of swelling and is now on restricted work as of 7/26/10.
Reportable only due to fracture. Employee was holding in the conveyor lever on the feeder when she felt a burning sensation between her shoulder blades (fracture to left 4th rib).
Co-worker accidently struck employee's hand with a hammer while removing a bucket pin.

Employee States that while he was bending over to pick-up the miner cable to move it out of the roadway, the rib rolled off and struck him on the (R) leg and knocked him to the ground.

On 05/25/10, a mechanic replaced the MXWM domestic water pump. He stood up to reach for a pipe wrench & felt a sharp pain in his back. Evaluation by the plant Dr. was a pulled muscle. Injury was classified as a FAC. On 08/04/10, MRI revealed emp. has a herniated disc. Emp. will receive further evaluation. On 08/11/10, this case was re-classified as an OER-RWC.
Injured was switching mantrips when he caught foot between rails.
EE reported that while moving calibration plates, he strained his hand. He reported months later that he had problems with his hand since incident, though he had continued normal work activities. Physician prescribed restricted use of hand.
Mechanic opened the right side of a double door to the atomizer feed pump pit and stepped inside. Several seconds later a short section of purlin with attached metal siding broke loose and fell striking the employee in the back of his hard hat and upper right back. This caused the employee to move forward at which time his right lower leg came in contact with a platform.
While changing a drill bit on a drilling machine, the employee accidentally activated the boom movement which caught his hand between the drill bit and stone surface. The result was a fractured 5th Metacarpal of the left hand.

Employee was setting a wear plate in the back of a Cat end dump, when plate slipped out of plate clamp and bounced and then landed on left foot.
While loading drums containing soil, EE and a Hycroft EE were attempting to place a drum between another drum and the side of the skid. The drum became hung-up on the section of E Rail track and caused the drum to fall foward and out of the drum picker. The drum ring landed on the EE's foot above his steel toes. This caused a fracture in the bones of his left foot.
In the process of changing the drill bit on a mobile bast drill, the drill steel kicked off to one side while the EE was holding the bit. The right pinkey finger of the EE was mashed between the bit and the steel frame work.
Installing Air Lines - EE was inside Manlift Basket, knelt down to reach pipe that was being installed, felt something tear in right knee.
Employee working at finish end of crushing plant, on Eagle Wash Screw, when employee slipped and struck knee.
Employee was holding onto a air lance while trying to dislodge a piece of buildup off the burner pipe. The piece broke free and jerked the air lance. The employee was still holding onto the airlance and strained his right shoulder. Employee attempted to condition the shoulder but was determined to have a torn labrum. Employee was anchoring a piece of equipment with a ""come- along"" when the lifting mechanism broke causing the item to strike him in the left thigh resulting in a contusion.
EE was preparing a shot on the bottom ledge when he heard a rock sliding. As he looked up he noticed a piece of stone sliding from a berm above him. While moving away from the face, the stone glanced off another berm below causing a rock to be projected out away from the face. The rock struck ee approximately three inches above the right knee.
Using a utility knife to remove a paint roller pad from the frame cutting the right forefinger.
Right shoulder strain. Employee was laying on his left side replacing feed wear plate. He reached overhead to position plate and heard a pop and felt a sharp pain in his right shoulder.

While performing a simons cone disassembly the injured party was guiding the cable used to pull the cone head around to unscrew the bowl assembly. He went readjust the position of the cable just as pressure was applied and 3 of the fingers on his right had became pinched between the cable and bowl assembly of the cone fracturing the tips of his fingers.
Employee was under railcar cleaning end caps. Felt ""pull"" in his lower back as he crawled out from under railcar.
Employee was climbing down ladder from Rubber tire rig CAT 834 to place a marking cone. When he started down ladder, his foot caught on the 3rd step and fell aproximently 4 feet to the ground, this resulted in a back contusion.

EE was changing feed chute rubber skirting. While repositioning himself, EE grabbbed skirting clamp that was hanging, knocked it down on his hand, breaking the tip of his finger.
Employee was helping perform maintenance on a conveyor belt. He put some tools in a service truck and was returning to get some more tools to return them to the tool room. As he was returning to the conveyor, he slipped and fell. Employee did not report the incident until the next day 05/20/2010.
EE was dismounting flatbed truck after releasing tie-down straps. Stepping onto flat unpaved surface- Twisted left ankle resulting in non-displaced fracture left foot.
Employee was removing chunks from G289 airlock with a rod. While employee was unplugging the G289 airlock; dust blew out of the pipe and into employees eyes.
Miner (mechanic) was directing operator of forklift in placement of boxes of dust collector bags as part of dust collector work. Miner did not pay attention to hand placement in close proximity to forklift tines. Forklift tine contacted miner's left index finger causing an avulsion on top of left index finger. Miner received stitches to close the avulsion of the left index finger.
Employee was opening a valve and felt a twinge in back afterward.
The employee was shearing plate steel on metalworker. Employee attempted to catch a piece of steel from falling and pinched and lacerated his right index finger.
Using an impact air wrench to install a bolt in a bush hog wear plate. Using left first and middle fingers to hold the bolt in place when bolt spun, lacerating both fingers.
Employee states he was welding on a refuse bin and slipped in the mud when relocating the bin. The employee jammed his right thumb as he was attempting to stop his fall, hyper extending his thumb. Employee sought medical treatment and worked 5 days on modified duty.

The employee was walking in a pedestrian walkway across main plant roadway and stepped on painted surface of walkway and slipped and fell. Rain had caused walkway to be slippery and the painted surface did not have anti-slip material added to the paint at the time of application.
While operator was operating a rubber tire dozer on a dump, he turned around to look behind him, as he brought his head back to normal position his neck popped.
