Accident type, without injuries

Four blasting personnel were exiting the open pit when the driver noticed smoke from under the hood. The truck was stopped and chocked and efforts begun to extinguish fire. MSHA was notified and fire was extinguished.

Shovel operator opened door to enter lube room. Wind blew door closed. Door latch was inoperative. About 45 minutes later, maintenance personnel opened the door from the exterior using the outside latch. Believe the employee was ""entrapped"" inside room for about 45 minutes. Appears snow melt and freezing temperatures froze the latching mechanism.

A non-injury roof fall occure inby spad #182. The fall is in the #5 entry and is apprx. 40' long X 20' Wide 6' thick.

Seven employees were descending down the elevator when a electrical storm was passing through the area. Wind shear knocked out the mine and building power and the elevator. Power did not come back on until 12:15pm at that time the persons were removed from the elevator. No damage occurred to the elevator and was checked by company maintenance.

The shaft hoist failed to operate which eliminates one of the two required escapeways.

Around 10:55am the hoist operator noticed a crack in the main block of the HPU for the #1 Hoist. Maintenance called to replace block valve. MSHA Notified at 11:25am. No need for evacuation, #2 & # 3 Hoists were still operational. Block valve replaced, test trip ran and #1 Hoist back up at 12:15pm. MSHA notified of corrective actions at 12:15 PM. (Incident # 1-101114264)

The crew was involved in muck cycle tasks when the rope on the assist winch failed due to a loading issue as the scoop transitioned onto the slope. This submission is by request of MSHA. For additional information please refer to the report prepared by Frontier-Kemper Constructors, Inc. pursuant to 30 C.F.R. 50.11.

a roof fall occurred inby an area that was under order between spad #179 to spad # 184. The fall is in the #3 entry and is approx. 70' long X 20' Wide--6-8' thick. All accesses to the fall will be breakered off as condition warrant. This was an unplanned roof fall.

The retainer ring grove on the fulcrum pin on the drum brake shoe will no longer hold the 1/16 thick retainer snap ring in place. The fulcrum pin and the two brake guide rod assemblies have not moved and are in their proper location

on 7 north section G2 panel the intersection fell in at 9 brk. The area was bolted with 6' tension bolts and 12' cable bolts. Top was laminated sandstone with a cutter in it noticeable after the fall. fall was aprox 12' and 17' wide. No ventilation affected and no injuries

The hoist lost power, and the monitor recorded (a field exciter over current). The hoist was immediately locked and tagged out. MSHA was immediately notified.

A roof fall occurred one break outby the face in the intersection in the #6 entry. The fall was approx. 20' X 30'. The fall was above anchorage, 6ft fully grouted bolts was used. This fall occurred after the area had been dangered off. It occurred at break 15. This was an unplanned roof fall.

Four haul trucks were started on fire. The fire investigator has classified this fire as a arson.

A roof fall occurred in the crosscut #4 to #5 in Southeast Main 80 feet inby survey station 959. The fall was approximately 60 feet long x 7 feet wide x 7 feet high.

The Crabapple elevator was inoperable for an extended period of time. There was no injury to persons.

Control operator observed large swings in the coal feed system. The system was shut down and restarted approximately 2 minutes later. Upon reactivating the system, the coal explosion suppression system activated. The suppression system detected an increase in pressure and activated working as it is designed. No employees in the area and no injuries reported from incident.

Three employees got trapped on the elevator at 11:05 AM. Schindler Elevator Service was called to evaluate and correct the issue. The employees were freed at approximately 12:15 PM. The elevator was thoroughly examined and evaluated. The elevator was returned to normal service at approximately 1:55 PM.

An excessive rain event washed out a roadway. The roadway is located at a remote location on the mine site that is not active and is restricted to light vehicles only during periodic site inspections.

When Hoist man moved skip above collar doors he noticed the high amp indicator light came on, he went to check the MCC room, when he got there he noticed smoke coming out of the vents, on further investigation he found the contacts for the 2300 volt system were burnt and melted.

At 7:10am the brakes on #3 hoist failed to release. After troubleshooting for 20 minutes (7:30am) the call was made to evacuate out #1 hoist. MSHA was notified at 7:40am. Troubleshot from 9:30-11:45am. E-stop had gone bad causing the brakes to not release. E-stop was changed out and a test trip was ran. #3 back up by 12:10pm. MSHA Notified of corrective action at 12:45pm.

There was a roof fall discovered in the first northwest mains at xc 4 number 4 entry spad # 2+40. Approximate size was 12 ft. in length 19.5 ft. in width and approximate height 5 to 6 ft. Pump cables and water line ran through the xc.

Hoisting equipment at the Pleasant Grove portal was not fully operational due to several electronic switches.

A roof fall was discovered by Mine Examiner and State Inspector while traveling old 2A intake cross cut 15 inby spad 23832. The fall measured 20 foot wide, 20 foot long, and 8 foot high. No injuries were reported as a result of the roof fall.

A roof fall occurred two crosscuts inby spad #7847 on the 6 East Panel. The fall measured 60' long x 20' wide x 5' thick. There were no injuries connected to this fall.

An unintentional roof fall occurred on the 5D, Entries 5-6 Crosscut 2. The roof fall measured approximately 25' in length, 18' in width, and 5' in thickness. A 103 (k) Order (8448054), was issued January 22, 2013, by MSHA Inspector.

The grapevine elevator stopped with w men aboard. The two men were on the elevator form 6:15pm until 7:20pm. The breaker for the over travel switch was re-set and the elevator ran and the two men were no longer trapped.

Found a roof fall during a pre shift in the 1st East butt. It was in the last open crosscut from #3 entry into both left & right cross cuts. It measures approx. 80' long 19' wide 5' to 6' high

A roof fall was discovered by an examiner in a worked out area of the mine on the 3rd Left Mains Panel, one break inby spad 1370. There was no interruption in the ventilation and no equipment of value involved.

The jumper lead on the hoist motor rotor at the connection point shorted out, causing a hole to be burned in the end of the rotor. The motor was removed and taken to a motor shop, disassembled and repaired. The motor was reinstalled and running on 01/27/2013.

An unintentional roof fall was discovered in the Rattlesnake Headings, Entry #4, Spad # 646. The fall measures approximately 20ft L X 20ft W X 6ft H. No injuries resulted from this occurrence.

A fall of the roof occurred on the 1st N.W. Panel #4 unit cross cut 38 #7 entry, measuring 15' length by 19' width by 8'-9' height.

An electric pump that supplies the gravity oil tank burned a fuse which tripped the safeties and shut down the hoist. Because of a faulty fuse panel the back-up pump did not automatically take over. The primary hoist was still operational and no one was stranded due to this occurrence.

The crane above the service hoist had an electrical problem in the drive unit. This unit is what allows you to let the hook of the crane up and down. We removed the hook and secured cables to the crane and put service cage back in operation.

During examination of the out by area of the mine that is return air course, a roof fall was discovered. No injuries to anyone, no damage to equipment, and does not conflict with air course. All three entry's to fall area was dangered off and timbers where set. The mine will continue to follow roof control plan.

A roof fall occurred in the #4 entry (belt entry) inby spad 94+38 along the Sturgis mains. The fall was approx 19 feet wide, 50 feet long and 12 feet plus high. The fall is located approx 7500 feet from nearest section.

Unplanned roof fall occurred at X-cut 12 in the 3A area in the #6 entry 1 X-cut from spad #57321. The fall was 20 foot long, 18 foot wide, and 8 foot high. The fall was crib and danger off and it will not be cleaned up.

people entering the mine found that the doors at the top of the Pleasant Grove portal elevator would not close. Mechanic was called to replace sensor strip on door.

During retreat mining we had a pillar fall occur that extended from the area where pillars had been mined in the normal cycle of retreat mining and extended outby where the continuous miner was cutting coal out of a pillar while performing retreat mining.

An unintentional roof fall was discovered during the weekly airway exam. The fall was located in 24 Zone, #4 Entry; 13 Block;Spad # 9339. The Fall measured 85 feetlong, 18 feet wide, and 7 feet high.

Roof fall was discovered in the 6th North West Submain Headings, xc-28, Entry # 7, (20+80). Approximate size of fall was length 25', width 18', and height 6' plus.

The dozer operator on shift was pushing coal to a stockpile reclaim feeder when the dozer became stuck on the slope into the feeder. Poor coal pushing practices created the unstable slope.

The rectifier for the elevator braking system shorted out on the 8 Haulage elevator. The elevator was non-operational for 42 minutes.

At approximately 11:45am the Shindler elevator became inoperable. The processor was rebooted and back in operation.

2 EMPLOYEES LEAVING UNDERGROUND AT 11:50 PM WHEN ELEVATOR TRIPPED OFF ON OVERSPEED. MECHANIC CLEARED FAULT AND ADJUSTED GOVENOR.

The #8 elevator would not unlock the bottom hatchway door when called. The shift foreman, MSHA, and DEP were notified. Shindler was notified and arrived at 8:30pm. There was a set screw loose on the coupler between the door lock motor and the door lock. The screw was tightened and adjusted. The car was tested and put back into service at 11:00pm

Due to an unknown electrical outage we lost power to the production hoist. All employees evacuated via the Koepe hoist with no injuries or other issues.

Kuhntown Elevator went down at 12:21 AM with 14 persons on board. Persons were off the elevator at 3:49 AM safely. Electrical contactor was replaced to correct the operation at the elevator.

The number one intake portal was blocked with rock when a portion of the highwall over the drift fell in. No injuries were received. All personnel were pulled from the mine.

A non-injury roof fall was found between the #9 and #10 crosscut of the north mains in the #6 entry. The fall was 6 to 8 feet high, 20 feet long and 16 feet wide. A breaker row had been set outby. 6' resin bolts were installed in entry.

A power surge was created because of a brief power bump that occurred from our power source (Avista Utilities). This caused fuses to burn in the hoist fuse bank and resulted in the hoist being shut down for 2 hours and ten minutes. There were 96 persons underground at the time. We still had 2 hoists that were operable.

A roof fall was discovered by the mine examiner in the intersection of No. 8 entry approximately 800 feet outby the dumping point at spad No. 2857. No ventilation, travelway or workers were affected by the fall. Fall measured approximately 20'x 20'x 8'high. Fall area cribbed and dangered off. Mine is non-producing.

A roof fall occurred in inactive workings in F-Seam, entry 7, crosscut 14 to 15 in the Southwest Main Returns. The roof fall in inactive works partially damaged an adjacent stopping.

The hoist operator noticed the temperature was higher than normal and called for maintenance. After maintenance arrived and evaluated the situation they downed the hoist for trouble-shooting purposes.

At 1:15PM a fire was reported in the MGO warehouse by plant operators that required both a Fire Dept.response & reporting to MSHA @ 1:28PM. The fire lasted beyond 30 minutes until extinguished @ 3:22PM. Extensive water damage of bagged MGO product from fire Dept.There was not a direct cause of the non-injury fire determined by the Woodville Fire Dept. or the company at this time.

An unintentional roof fall occurred at #95CX # 4 entry spad #667 of the Alma north Mines. It was 10' long 15' x wide x 6' high, a clean up plan was submitted and approved.

At approximately 10:00 am a roof fall was discovered near survey spad 2962 blocking the approaches to DPP 52.

A roof fall occurred in the 3rd Main East intake between x-cut 30-31 in the pillar line #7 intake entry. The fall meas. 25'x6'x19' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension glue bolts and was mined in 3/08. There were no injuries and no EQ. involved and ventilation was not affected.

An unintentional roof fall occurred in the intake entry of 1 J miner section. The fall occurred inby the #23 xcut. The fall measured approx 50 ft long by 16 ft high and 9 ft high. No injury or equipment damage as a result of incident

An unplanned roof fall occurred at x-cut #2 on 4C belt. The fall was 18 foot wide, 20 foot long, and 6 to 7 foot high. The fall was cleaned up and the area was rebolted.

A non-injury mountain bump occurred inby the 5N longwall. Additional floor heave and rib falls. Some damage to ventilation controls, outby to x-cut 19 #2 and #3 entries. Same area as 12-2-2012 event. The longwall face was inby 17 x-cut.

A fire occurred at 9:15 p.m. on the surface at the upper rock bin at head frame for rock belt PB8B, where a drive roller lagging and belt caught fire. The fire was extinguished using fire extinguishers.

EE was a self-employed individual who was subcontracted by T & T Energy to perform welding work. EE was performing welding work on the T & T Energy, Begley #1 jobsite and while working experienced fatal blunt force trauma to his head. There were no direct witnesses to the accident and the accident is currently under investigation.

Employee worked as lead maintenance mechanic. Believed on morning of accident, Employee went to area Kiln 2 Preheater, Ram 4 to respond to work order. Employee had two-way radio. No witness to accident. Unknown whether employee was inspecting work or performing work on the No 4 ram. Initial examination suggests Employee was injured when ram activated. Ram was not locked out.

Struck by falling object

While retrieving samples from quarry face, employee was struck on left shoulder by a rock, resulting in fractures.

Miner ""ran"" up the haulage to get out of the way of a haul truck. First aid applied. Went to Dr on 2/18/13 for continued pain, possible hernia.

Caught in, under or between running or meshing objects

Employee was running drill steel into hole using steel puller, had hand on drill steel, when hand got to puller, didn't remove hand in time, finger got into puller wheels. Smashed employee's right index fingertip, amputated from bottom of nail to tip of finger.

Over-exertion in lifting objects

Employee was putting dirt and dry grease into a small steel bucket that had an approx weight of 10 lbs. When lifting the bucket from the ladder with his right arm he felt a pull in his lower abdomen. He was evaluated on 1-22-13 and diagnosed with a hernia. He returned to work full duty.

Employee was bolting at the bottom of the slope. He was trying to get a plate up over the drill guide when it caught his finger.

Struck against a moving object

The EE stated he ran into a hole in the road at the #1 switch back curve. The EE stated he felt pain in his lower back. He sought medical attention that day & returned to work under modified duty until the 25th at which time he was placed off work. EE missing work on 1-25-13. *On 3/19/14 the employee was awarded 5% Permanent Partial Disability as a result of the injury.

Employee was lifting an empty wooden pallet and felt a sharp pain on his side.

Caught in, under or between a moving and a stationary object

While moving a 400 pound sump pump, employee attempted to level the load, when the lifting eyes broke sending the pump back to steel table, employees left pinkie was struck causing a laceration.

Over-exertion in pulling or pushing objects

Employee was sliding 4 inch pipe through a masonry stopping when the pipe stopped suddenly when it hit a bolt block. It fractured his left wrist.

He was splitting stone and had pain in his back.

Employee was installing a Rub Rail on a the #4 Shuttle Car, when the rail fell on top of left foot.

Over-exertion in lifting objects

Employee attempted to lift a piece of metal that was obstructing his path to his tool box. In doing so, he strained his back and finished his shift. He continued to work for the next couple of days before deciding to seek medical treatment and is under care of his family physician and anticipates returning to work within the next few days.

Struck by falling object

Employee was reaching for a roof bolt plate when a piece of draw rock fell, striking his left fore finger. He was taken to ST. Joseph Martin Hospital and received X-rays. It was determined that a small piece of metal from the plate had penetrated his finger and that he would receive surgery later to have it removed.

Mechanic was cutting a relay electrical connector in half with a personal pocket knife (in a tractor, behind the dashboard) the knife went through the connector into his left index finger cutting a portion of his skin off of the finger.

This description/recount is told to me by an ee. According to that ee at approximately 1:00pm he along with another ee were assisting the mechanic who was doing maintenance on the screen. EE and another ee were pulling the motor up. EE got his (L) hand caught between the cable and the pulley. EE was taken to LaFallette Medical Venter ER.

Employee was bolting #2 right, as he was placing a bolt in the hole he felt pain in his right shoulder.

Employee alleges that on 01/31/13 his right knee ""exploded"" while walking and carrying something for an electrician. He did not complete an accident report. He notified his supervisor on 02/01/13 that he sought medical treatment. The worker's comp paperwork employee completed on 01/31/13 reports a DOI of 12/19/12.

The employee stated while installing his middle roof bolt in Entry #5 a rock fell inby supported top startling him. While trying to move away his left foot was caught between some rocks on the mine floor. While moving his left foot twisted spraining his ankle. He stated to his supervisor he felt a burning in his left ankle. He did not report to work on his scheduled Monday shift.

Employee was working on a dozer in the shop, as he climbed off the dozer he felt his right knee lock up. He was unable to straighten his right knee.

While moving the roof bolting machine from the operator's deck the employee pinched his arm between the machine and coal rib.

Employee was walking and stepped into a depression in the mine floor and twisted his right knee.

Employee was moving longwall monorail. He bent over to pick up one end of the monorail and it was stuck in the mud. He felt pain in his shoulder.

Fall to the walkway or working surface

Employee was walking past flocr bin when he stepped onto slick spot on floor his feet flipped out from under him he. Fell backswords onto floor hurt his upper back and neck.

Over-exertion in lifting objects

Injured was lifting belt with a Slate Bar, and felt pain in his back. He finished but went to the Dr. later in the day. Was told to take 3 days off.

Struck against stationary object

Employee stepped on lose rock while checking in vehicle at guard house. Employee's ankle rolled causing a sprain.

Employee was going to change batteries on the scoop. As he was getting out of the scoop, he twisted his left knee.

Fall onto or against objects

Lower Tower Hill: the site had large frost slabs with snow covering them. We could not see what was under the snow. I stepped on a rock. The rock slipped and rolled out from under my foot. I fell backwards on my left side. My lower left side came in contact with a sharp boulder and knocked the wind out of me. The walk-in clinic x-rayed and diagnosed a bruised lower left rib.

Fall from scaffolds, walkways, platforms

Employee stated he went to step off of crosswalk at a transfer and forgot that he had removed the ladder and fell 5'5"" to the ground.

Caught in, under or between a moving and a stationary object

Mechanic was getting metal angle iron from rack. As he lifted the 20' long piece he positioned his hands and body improperly. An unexpected shift of the other angle iron pieces resulted in the tip of the middle finger on his right hand being crushed against the metal rack. Tip of the finger had bone crushed nail was damaged and skin split on side of finger.

Employee was reclaiming a Kennedy wall when a rock fell from the top and struck him on his right hand and ankle.

An EE for Contractor ID# 1VY Alex Energy, Inc. was on a reel trailer rolling fiber optic cable onto a reel, when he slipped and fell. Area was wet and muddy. The EE fell striking his tail bone on metal eye that was on top of a stabilizing jack.

Over-exertion in pulling or pushing objects

Employee stated that he was down on his knees with a come-a-long ratchet wrapped around a pipe when he used one hand to lift the pipe and the other to pull the ratchet from under the pipe. This resulted in a lumbar spine strain.

Changing out a bottom roller on a conveyor belt and hurt his back.

While doing maintenance on a Euclid R-50 Hual Truck and using a 1'x1'x1' (+/-) block of wood as a step turned to step off/down from the block and rolled right ankle resulting in a sprain.

A contract employee was reclaiming a Kennedy wall when a rock fell from the top and struck him on his right hand and ankle.

Struck against a moving object

On 1-29-13 employee was operating motor grader when blade hit a rock causing the operator to jerk her back. On 2-26-13 employee begins losing work days causing injury to be reportable.

While retrieving vent tubes from the face area of the #1 entry in 21st West, the employee twisted his left ankle. EE was examined by a physician and the results of the x-ray were negative and the EE was released to return to work. The next day he return to the physician and was given a work release excusing him from work for that day.

Fall from machine

After checking the coolant and air filter box for the pre-shift inspection of the Jumbo Drill, she slipped and fell to the ground when dismounting the drill. The employee landed on the her left side and injured her left lower back.

Struck against a moving object

Employee trammed his diesel locomotive into a parked shield that was on another locomotive sitting on the track outby the Longwall recovery switch resulting in him sustaining fractured ribs.

Struck against stationary object

The EE stated he was walking beside the parked CM when he stepped on the miner cable and his left foot slipped of the cable causing his right leg to land awkwardly and his knee twisted. EE started to miss work to the injury on 1/31/13 and RTW on 2/4/13.

Struck by falling object

Employee states that he pulled the ventilation curtain down to install a center line and was struck by a piece of rock that would weigh 35 lbs. The rock fell approximately 11 inches and struck employee on right shoulder. Employee had an xray c-scan and an mri and all were negative. Employee complains of numbness in right leg and difficulty in walking.

The EE stated he was walking along #2 East Belt when he slipped in a hole of water and strained his left knee. EE started missing on 1/30/13 and RTW 2/1/13.

As the EE was traveling from his car to the shop he slipped on ice, spraining/straining his lower back. This was not reported until now because EE didn't seek medical attention until 2/4/2013.

EE stated he was walking by the scoop and stepped in a hole causing him to twist his right ankle. EE finished his shift on 1/28/13 and reported for work on 1/29/13 but was unable to work the full shift. EE sought medical treatment and was placed off work for a strained knee.

Employee was helping remove the pan line approximately 8 to 10 feet from the west maine raise bore shaft when some red clay mud fell down the shaft striking the pan line and employee left shoulder. As the employee went to get out of the way he fell down hurting his left ribs and shoulder.

The miner was working on installation of a new dust collector. He had to reposition his safety harness by standing on a ""X"" brace of the dust collector to adjust himself. He slipped on the angle iron of the ""X"" brace about 8 ft and his right foot hit the end of the brace. X ray revealed that he had fractured bone on his fifth (smallest) toe of his right foot.

Emp. reported to supervisor he had fell from 18 conveyor tripper car and fell on his left elbow. His left arm was in pain. Left leg had a minor scrap from step.

EE was on top of the tank to manage the position of the pump. The static thickener rake pulled the pump discharge hose which caused the crane boom to spring & hit EE's leg.

Employee allegedly incurred tennis elbow while moving two pieces of steel. (1 angle 3X3 X 1/4 in thick x 8 ft and 1 flat bar 4 in wide x 3/4 in x 8ft. The employee claims that he hurt himself 10 minutes into job. The employee was assigned to stack steel onto a metal rack.

Employee was pulling up the cell 3 barge pumps and the cable being used snapped and hit employee in left arm. Contusions from bicep to forearm.

Hose came loose from air wrench and struck employee above the left eye, lacerating the scalp and knocking off his safety glasses.

Over-exertion in lifting objects

The employee was emptying barrels that fill up during production and employee strained lower right back. Employee decided to wait and see if it would get better on its own. The injury did not get better so employee made an appointment with Dr. on 1/31/2013

Fall to the walkway or working surface

Injured was exiting the pack house silos on the south side. His left foot did not clear the concrete blocks (used to stop water from entering these silos) and he fell to the ground injuring his left knee (torn ligament)

Caught in, under or between a moving and a stationary object

EE reached around a guard to clear material conveyor without following proper lock out tag out procedures. Hand was caught between the tail pulley and idler conveyor. He was able to pull a stop cord stopping the conveyor, but remained with his hand caught. He had to wait assistance to be removed from the conveyor. His hand received abrasions as a result of this incident.

The man slipped and fell while walking in the mine and injured his shoulder when he attempted to stop his fall. The man continued to work his regular duties until 1-29-2013 at which time he went on a restricted duty status.

Struck against stationary object

The man was tramming the personnel carrier up the travelway when it became stuck on a rock. As he was traveling in reverse to get off the rock, the brakes failed to stop the carrier immediately and he backed into the rib and struck a post which hit him on the hand.

Employee misstep while descending silo stairs, fell and twisted right leg.

The employee was placing ore control visual targets,when he slipped and fell backwards,landing on his backside and hitting his head. The employee was taken to the hospital where he was given a full duty release. The employee was still experiencing dizziness. He went back to the doctors on 1/14/13 and was put on restricted Duty starting 1/15/13

WHILE LIFTING A STEDMAN WEAR PLATE INTO PLACE, EE LOST HIS GRIP. THE PLATE FELL 2 FEET ON HIS LEFT PINKY FINGER.
