Accident type, without injuries

On 10/31/2015 at approx. 10:30 am a roof fall above the anchorage was fond on the 4 North track at spad #24+47. The area had been mined in 2009 and was supported with 5 ft. fully grouted bolts. There were no injuries or property damage. The fall will be cleaned up and the area re-supported. The mine was idle at the time of discovery. Normal weathering may have been a factor.

At approximately 4:30pm the service hoist went down due to DC fault trip. At approximately 6:00 pm it was re-set and the service hoist was put back in operation.

While checking a pump in the return at approx. 9:25AM a certified examiner identified a roof fall. The fall was located in the 2 section return #1 entry about 80 blocks outby the section near spad#3433. The fall measured 17'W X 16'L X 7'H and was supported with 5' fully grouted bolts. The area has been dangered off and timbers set. The area will be cleaned up.

Switch on the bottom elevator doors was misaligned and would not operate. Switch was re-aligned and tighten down.

On 10/28/2015 a roof fall was discovered in the 3 West Mains air course where miners do not normally work or travel. The fall was located at break 25 in a cross cut from #8 to #9 entry.

A roof fall was discovered on the 3 West Mains at break 31 in the intersection of the #6 entry. The fall was located at spad 2185.

A roof fall was discovered in the G-District Bleeder in the #1 entry, #20 crosscut of the G-5 panel. The fall obstructed the travelway to examine BEP-7G1 and BEP-7G2.

Electrical components failed in PLC cabinet that controlled the dynamic brake for #6 hoist motor. MSHA notified at 4:55 pm. Repairs made and hoist was back in operation at 8:00 am on 10/29/2015. MSHA notified at 8:01 am.

Bearing failure on generator that supplies power to amplidine to #2 Hoist, at 3:05 pm. MSHA notified at 03:17 pm. Repairs were completed and hoist returned to operation at 5:42 pm. MSHA was notified at 5:43 pm.

An unintentional roof fall above anchorage was discovered at approximately 9:00 AM. The fall occurred at 14 Tail Gate Break 114, 1 thru 3 crosscuts. There were no injuries nor was ventilation impeded.

While making a preshift exam at this idle coal mines the mine foreman found a roof fall in #5 entry just in by spad 64 measuring approx. 5' in height 20' in width and 20' in length. Said roof fall did not impede travel or ventilation.

While conducting on-shift exam of the pumps in this area of the mine, the examiner discovered an unintentional roof fall in the No. 3 entry of 26 zone one break outby spad C439. The roof fall measured 14 feet wide, 25 feet long & 5.5 feet high. A ""wild"" coal seam and irregular roof was present at the anchorage zone of the 5 foot fully grouted torque tensioned roof bolts installed.

Roof fall was discovered on early pre-shift, before normal shift began. Fall was located at x-cut 58 in the intersection, outby section in #4 entry. Approx. 20' wide x 18' long x 10-12' high. Fall found at 9:45pm, MSHA notified at 9:51pm.

A fall of roof occurred in the Southeast Submain, crosscut 141-142, entry # 6. The measured approximately 18 feet wide x 45 feet long and the thickness is unknown.

On Friday October 23, 2015 at approximately 6:00pm a roof fall was found in the North Main #4 entry intersection at crosscut 32. The roof fall is approximately 25 feet long by 25 feet wide and approximately 10 to 15 feet high. The roof fall is not located in the weekly examination route entry.

Found a small smolder of coal on the east side of the ramp in the coal storage building, it was considered totally out at 10:52 pm.

On 10/23/2015 a roof fall was discovered in the 3 West Mains at break 31 in an intersection. The fall was located in the #5 entry at spad 2196.

Unplanned fall discovered reportable of brow of southern approach to 2 belt trench at the drive. Fall was above anchorage and 4' high, 8' foot long, 16' wide. Investigation discovered brow separated in area of damp, deteriorated clay material. Fell to trench excavation height. No problems with training, PPE, mining machines or compliance issues. Area mined in 1965.

Miner went into 5200 motor barn to retrieve motor to transport crews to their workplace. Take motor out miner drove by charging station and noticed arcing coming from cable connection to spare battery. Miner tried to fight with fire extinguisher.

The hoist car went down at 1:40am and was put back in service at 2:15am. The ""E"" stop was in and needed pulled out.

Found an unintentional roof fall at Break #39 in the #2 Entry parallel with the #4 Belt line. Roof fall dimensions were approximately 20' long by 50' wide and 8"" thick.

Elevated temperatures were discovered in a rotary feeder in the coal system. The fire supression system activated; and all material was pushed out of the rotary feeder. No flame or smoke was identified. One of five vents in the South dust collector ruptured when the door to the dust collector was opened. This event is in connection with citation number 8900222.

The elevator at the #9 Portal had a malfunction due to slack in the Governor Rope. There was no one on the elevator at the time of the incident. Outside electrician was called and the elevator was up and running at 10:30PM.

A FALL OF THE ROOF WAS FOUND ON OCTOBER 19, 2015. THE FALL WAS IN AN INTERSECTION ONLY AND WAS REPORTED TO BE 3 TO 4 FEET THICK. ROOF BOLTS WERE SEEN STICKING UP THROUGH SOME OF THE FALLEN ROCK. THE FALL IS ONE BREAK INBY SPAD NUMBER 1965 IN A NEUTRAL/NONE ESCAPEWAY ENTRY. THE AREA HAS BEEN MADE SAFE BY CRIBBING AND DANGERING OFF.

On 10/18/2015 while a shift supervisor was en-route to the PS 500 substation to investigate why the main facility and the draggle had lost power, a fire was noticed under the 25KV power line next to the Turlington haul road south of Browns creek on the west side of the road.

Roof fall occurred 4th east rooms in entry 3 crosscut 2 no equipment or people involved no damage to return stopping there were no timbers or cribs in this area there was extra support straps and bolts. The fall was 45X35 and 15 feet high. This Area was originally bolted with 8' TR bolts and 12' cable bolts area was dangered off and timbers set.

The Oak Springs hoist was out of service due to a loose shaft on the blower motor. The hoist was put back in service at 12:35 PM.

While attempting to abate a citation for mandoor distances at 10-17 between belt and old loaded track a roof fall was discovered between square sets and the stopping the fall was determined to be above anchorage. Support will be added to create access to terminate citation.

At 5:50 am on 10/16/2015 while checking the water level we found a roof fall in #2 entry (return) at 72 break. The fall was 30' long by 20' wide and 36"" thick. There was 36"" roof bolts and they came down with the fall. This is an untraveled area.

The Cameron elevator was taken out of service for a damaged sheave wheel There was no one entrapped we took it out of service and due to item 11 of immediate reportable events the elevator was reported

An unplanned ignition of combustible gases in the feed end of the #3 kiln occurred.

Electrical problems, evacuation started at 4:55 p.m., completed at 5:05 p.m., MSHA was notified at 5:06 p.m. Repairs were made and completed, at 6:45 p.m., the hoist was tested, and miners returned underground at 6:50 p.m.

Rock Fall occurred at spad 1005 neutral beside #7 belt. Rock was 20'x 22' from 0 to 42"". Area was cribbed off and did not impede travel or ventilation.

Cage would not go to mine level. Evacuation started at 5:20 p.m., completed at 5:25 p.m., MSHA was notified at 5:27 p.m. Timbers were greased and completed at 6:20 p.m., and miners returned underground at 6:30 p.m.

#30 Hoist at Fletcher went down at 3:45 pm on 10.14.15 due to an electrical issue. MSHA was called at 3:30 pm. Electricians were called for repairs. Repairs were completed and hoist was released back into service at 5:17 pm, MSHA notified at that time.

Roof fall. 2nd left submains 120 ft outby SS# 2646 at right bleeder. Fall was 5000 feet from working section. Fall was 6' high 18' in width and 80' in length. Entrance is dangered off.

A roof fall (66'L X 9'H X 18'W) was found on the old 6-North Track outby spad location 7810. This did obstruct the walkway but did not involve any other compliance issues. No equipment was involved and no other 50.20 criteria applies. The fall approaches will be supported and walkway rerouted.

On 10/13/2015 a roof fall was discovered at Brk 29 on the 3 West Mains in the #8 entry.

On 10/13/15 3 employees were stuck on the elevator at the Grapevine portal from 5:50 to 6:23 pm for a total of 33 minutes due to the door switch, the switch was repaired.

During a pre-operational inspection, the hoist operator discovered that a crack in the load line drum caliper brake assembly. The hoist was immediately taken out of service and MSHA was notified.

Maintenance Supt walk thru hoist house, heard the MG set squealing, had Hoistman shut the MG set down. Investigated - electrical brushes came apart. Takes one hour to stop MG set from spinning, repaired, up and running in 10 minutes.

An unplanned roof fall occurred at this operation on the 8th SE Panel near spad #9994 at break #38 in the #1 entry on 10/12/15.

A production Laborer was entrapped in the pre-heat elevator from 4:20-5:35 PM. The elevator repair company was called and they arrived to retrieve the employee. Employee was not injured and radio contact was maintained with the employee.

At 9:45pm a person on highway 782 called 911 to report a grass fire in the Oak Hill Mine DII Area Reclaim. Fire departments responded and extinguished the fire at 10:25pm. Approximately 1 acre of land involved. No injury or equipment damage. Cause: Lightning strike.

On 10/12/2015 a roof fall was discovered in the 3 West Mains air course where miners do not normally work of travel. The was located at break 31 in a cross cut from #3 to #4 entry.

The Metz elevator experienced a door fault error and engaged the safety brakes, entrapping 2 miners for 40 minutes approximately 175 feet from the top. The safety fault was reset and the elevator tested numerous times but would not reproduce the fault. No problems with training, PPE, mining methods or compliance issues.

Elevator service was disrupted for 4 hours due to a fault in the electrical main breaker. Service was restored approximately 1:30am on October 12, 2015.

Roof fall occurred in the # 4 entry 25 x-cut 6 seam. The fall location is in the entry at 3015 West to 3055 West and the dimension is 9' high x 40' long x 17.5 wide. A action plan was submitted to MSHA. The fall will be referenced as 6C.

A roof fall occurred out-by the 002 section on #11 belt between #208 and #209 x-cuts. The fall was 16 feet wide by 15 feet high by 45 feet long. The fall was reported because it was above anchorage. Approaches were re-bolted and cable bolted. Fall was cleaned up and a square set was installed according to our roof control plan.

On 10/9/15 a roof fall was discovered in an air leg that is in common with the Main West Belt between 29.75xc-30xc. The fall is 70 feet in length, 17 feet in width and is approximately 9 feet in height. The area was initially bolted with 8' point anchor resin assisted roof bolts. Weathering of the roof was determined to be the cause of the accident.

The elevator door became out of adjustment and would not allow the elevator to function properly. The doors were adjusted and the elevator placed back into service at 8:40 am.

A roof fall was found in our M4-Mains at crosscut 71 in the haulage road. The fall was not located in an escapeway, it did not impede passage of miners, affect ventilation or cause injury to miners.

A roof fall occurred on the H-2 belt line at #35 crosscut. The fall was approximately three foot high and twelve foot by twelve foot in length located towards the tight side of the belt.

A roof fall was discovered on this date. The fall was located in a crosscut at break #10 between the #4 and #5 entries, on L-3 left panel. The fall measured 20' by 20' and was 8' in thickness. The area was originally supported with 6' torque tension bolts. The fall was cleaned up and re-supported. There were no workers involved nor was ventilation affected.

At approx. 2:30AM the service hoist lost power due to a rat chewing a lead in too an causing a ground. The hoist was down approx. 70 minutes. No persons were trapped or injured. All employees were evacuated to the surface in a timely manner.

A roof fall was discovered at 1:43 pm. The fall of roof occurred in the 1st South Submain, crosscut 40, entry # 1. The fall measured approximately 18 feet wide x 20 feet long x 6 feet thick. The area was supported with 6 foot fully grouted roof bolts.

The east production skip did not fully dump on surface. The east skip returned to the mine and was loaded a second time. Due to the additional weight the skip was not able to be hoisted. The production hoist was not avaliable until the additional salt was removed.

While inspecting the SRM 3374, a miner came across a ground failure. Upon initial inspection, it could be seen that a large slab of rock failed, several feet below the springline, along the right rib of the drift. In addition, a large amount of unconsolidated material fell out as well.

The Crabapple elevator was not operational for an extended period of time. The ABB relay which is a field current that switches the breaking system from AC power to Battery backup failed due to under size. No one was injured as a result

Following a temperature and pressure increase in the bag house, five vent doors opened as designed. Investigation is still ongoing regarding the circumstances underlying the temperature and pressure increase.

While baling hay, the right rear roller bearing became hot. The heated roller bearing contacted dried hay causing a grass fire. The fire lasted about an hour.

A roof fall occurred above the anchorage point at survey station 57+35on the mainline near the river portal. The fall was 12 feet long and 10 feet high and the width of the entry. The fall occurred due to the age of the area, weather it didn't involve any compliance issues it did impede travel, no equipment was involved no other 50.20 criteria applies.

2nd S.Panel#2 Entry x-c9, roof fall was found intersection. Around 45ft x 25ft x 8-10ft hi in return airway where no one works or travels. This is same area last fall at x-c8 happened. The area was considered bad 9/1/2015 and lifeline rerouted extra support installed. Area originally bolted 8ft full grout rebar bolts & cable bolts in intersection. The ventilation not affected.

The weekly examiner found an unintentional roof fall inby the 4 Right EP. This area is not travelled or examined. The fall did not effect any other travel or ventilation.

When swinging a sledgehammer at a rock, missed the rock causing hand to contact the discharge chute that the rock was lodged in, causing a laceration to the finger and cutting the tip of the finger off.

Over-exertion in lifting objects

Employee was putting 25lb bags of product into boxes and strained left abdomen.

Struck by falling object

EE resealing track adjuster lock cover on CAT D11T unit #1601, as the access cover was removed oil leaked out, the EE was attempting to wipe up oil with one hand and hold the cover with the other. Cover slipped from his hand, as he grabbed it the cover fell to the ground smashing his left hand between it and the concrete floor. Per phone call ring finger amputated at 2nd joint.

RED LINE CASE unsure of work relatedness, investigation continues employee claims hernia, no specific date of occurrence, states happened in 2014 but did not file injury with Medical department. He did come to medical but it is documented the he wished to speak with union prior to reporting, Employee did not return to file injury report

Over-exertion in lifting objects

EE is not sure of the date of the accident, but did not get reported to the company until 10/14/2015. EE was cleaning in the lab and he aggravated his groin area while lifting a box. Went to the doctor before reporting to the companying and doctor said he has a lingual hernia.

Caught in, under or between running or meshing objects

Employee was unchoking a screw conveyor with a poker bar when his arm became entangled in the screw.

Caught in, under or between a moving and a stationary object

Employee was pulling linkage pin on a 980G Loader. Pin shifted smashing/pinching tip of left little finger.

Employee was positioned on a long/wall support pontoon to remove a relay bar bolt while the AFC panline was being dismantled/removed outby him. As the outby section of pan line was removed, it created a jarring motion of the pan line where the employee was working and struck the employees knee, causing pain/injury.

Injured was bolting top when a piece of rock fell from the top, striking him in his upper body, pushing him to the ground. He was setting up to install the outside bolt. After emergency room visit, was reported that nothing was broken, but he would be sore.

Fall to the walkway or working surface

While carrying a 2"" air pump in the bleeder walkway near 21W tap he stepped on a section of slick line and fell backwards, landing on his back. The location where he tripped was dry. He is experiencing localized pain in the upper lumbar and has been referred to a specialist. No problems with training, PPE, mining machines or compliance issues.

Caught in, under or between a moving and a stationary object

A fully trained maintenance helper had turned a shield off. He was standing on the shield that he believed had fully functioned. He started the shield and it lowered on him. Another employee raised the shield. The employee was trained as a shieldman.

Over-exertion in wielding or throwing objects

Swinging sledge hammer to tighten crusher nut.

Struck by falling object

Employee was backing out of the loader compartment when a piece of stone from in between roof straps struck him on the back.

Over-exertion in lifting objects

After handling a miner cable, the employee felt pain in his back. The incident occurred due to the ee's improper lifting and handling of the cable. The ee had on the appropriate PPE, was proficient, properly trained and has a proper attitude. The incident did not involve the mining system or mine equipment nor any aspect of compliance with rules or regulations..

Caught in, under or between a moving and a stationary object

Worker was helping to clean a conveyor spill. He picked up a rock to place it on the belt and caught his finger between the rock and other material on the belt.

Over-exertion in pulling or pushing objects

On 11/3/2015 EE reported to Supervisor, that appx a week ago he hurt his right shoulder while removing a pin from the steering jack on the highwall miner with a slate bar. He didn't think he was hurt at the time but has experienced pain since the incident

Employee was pulling a Johnson buggy into the shop and the tires were muddy. One of the tires picked up the rope that was hooked to the garage door rolling it around the tire. Door was pulled down onto employee's back.

Employee was loading 8 ft. timbers on a 4 wheeler and felt pain in his back.

EE was walking towards power center. EE slipped and fell, left leg twisted behind EE. The weight of the EE's body landed on left leg.

After washing down haul truck employee slipped on wet surface causing groin strain.

Employee complained of work related back pain prior to reporting for work. Employee cannot define an event, or cause related to the injury.

The employee was attempting to place a hook on a shield when a small rock struck him in his right hand. The following were not a factor: rules, regulations, skills, proficiency, training, protective clothing or protective devises on equipment. Attitude may have been a factor. Miner did not start to lose time until 11-2-15.

While employee was scaling rock in #2 entry on #1 Unit he moved a smaller rock dislodging a larger rock striking him on the top of the head pushing him toward the return side rib.

Injured was roof bolting, adding a steel extension, when the cutting tip contacted the roof causing the joint of steels to bow and break, striking left thumb causing a fracture.

Employee was coming down stairway when he slipped and lost his footing. He grabbed a hold of handrail hurting his left wrist and middle back.

INJURED HAD CLEANED GOB OUT OF UNCUT FACE. HE SET THE MINER OVER TO GET HIS INITIAL CUT. A PIECE OF ROCK FELL OFF OF RIB, STRIKING HIM IN THE LEG. THE WITNESS GOT THE ROCK OFF OF INJUREDS LEG.HIS LEFT LEG WAS BROKEN IN TWO PLACES.

Employee was installing a rail tie, Using a scoop to raise the rail and a slate bar to install the tie, The chain on the rail causing it to fall. The rail and slate bar struck employee's right foot

Contact with electrical current

The individual was welding with a DC Welder in a tight area when the left side of the neck came in contact with the stinger and received an electrical shock. This did not involve compliance issues, no equipment involved. Personal safety equipment being utilized. This did not involve job proficiency, or attitude. Individual went off work for surgery 1/21/2016.

A fully trained loader operator bent over to pick up the loader cable and felt a pop in his back.

Miner was walking beside a trailer, a fine piece of medal was sticking out from side of trailer, medal was a ling splinter type of scraped material, still attached to trailer. It was stuck in finger where they had to cut the material from the trailer to free the finger.

Over-exertion in pulling or pushing objects

Individual not concentrating while moving the track rail bender along the rail when it became stuck. When pulling on the bender he felt a pop in the right shoulder. This did not involve any compliance issues. It did not involve jobs skills or attitude. Personal safety items were being utilized.

Fall down stairs

While employee was going down stairs, he slipped and fell, hitting left knee.

Struck by falling object

Employee was helping to get a 60"" top roller out of beltline, lifting it over their heads. The roller got stuck so they pulled it with a slate bar and when it came free it struck him on the knee. The roller was too close to the top to use a chain ratchet.

Over-exertion in lifting objects

Employee was unloading ventilation tubing from shuttle car and felt pain in right shoulder. Employee was examiner at ER and was released to return to work. Employee worked his next scheduled shift and then followed up with his family physician and taken off work pending MRI diagnosis. First day missed for this injury is 10/29/2015

Employee was moving a piece of metal to be cut when he felt pain in his back.

Employee was tramming the roof bolter past a parked shuttle car. The bolter slid and caught employee's leg between the bolter and the shuttle car bumper.

While in the process of re-railing a supply car the car shifted and struck employee in the left lower leg, fracturing his tibia just above the ankle.

EE WAS IN A MANTRIP GOING TO HIS FIREBOSS RUN WHEN MANTRIP JUMPED RAIL AND RAN INTO THE RIB AREA. IN INCIDENT, EE RECIEVED A CRACKED RT RIB AND A BROKEN TOE. EE CONTINUED TO WORK THE REST OF HIS SHIFT. EE CAME INTO SAFETY THE NEXT MONDAY, 10/26/2015, AND FILLED OUT AN ACCIDENT REPORT. EE WAS TAKEN OFF WORK BY DR. UNTIL A MRI CAN BE TAKEN. FIRST DAY OFF WAS 10/28/2015.

Employee states on the 24th of October, dispatcher was getting out of her chair in the office and twisted her knee. Employee reported to safety department on the 29th of October and went to hospital from work and returned that date with no release until the 2nd of November. Employee states felt major pain and burning to knee.

Employee was walking with tools when he stepped off a 6x6 timber at the dewater location and twisted left knee.

Employee finished loading a round & while attempting to remove the air hose from the anfo pot that was in the bucket of a 4 yard mucker, a rock dislodged from under the bucket causing it to contact the top of the employee's foot, resulting in a metatarsal fracture.

Employee was walking and stepped into a hole and felt a sharp pain in his left knee. Accident was not reported, and employee continued to work until 11/02/2015 at which time employee experienced another incident with his knee while attempting to enter the deck of an underground forklift. His first lost day was 11/03/2015.

Employee was sitting in passenger compartment of CO #51 mantrip which was located at the mouth of the hole waiting for clearance. While the mantrip was sitting still, it was struck from behind by CO #84 mantrip. Employee stated that this jarred him causing pain to his lower left back. Employee was stepping off railcar ladder. He missed the bottom step with his right leg his left leg was caught on the side sill grab iron and he hyper extended his knee. On October 20th he received an MRI. On October 29th MRI results showed a complete tearing of his ACL and MCL. Employee is off work until ACL an MCL are repaired.

After arriving at the unloading point the employee got off of his Locomotive and began disconnecting safety chains from between the four Locomotives. Employee was climbing down from a ladder that was used to gain access to the top of a shovel bucket job. As he descended the ladder, it slipped out from underneath him. The employee was operating the 953 mantrip traveling towards the elevator bottom when a piece of draw rock fell and struck his left hand while it was on the brake lever. A fully trained miner was mudding a wall in 15 west the b-bond dripped inside of his gloves and irritated his right arm to his wrist the miner was not wearing the proper gloves for this job and was not wearing long sleeves either.

Employee was setting a stone on a feeder conveyor of the splitter and pinched his finger tip between the stone and the conveyor frame.

Employee when working became disoriented, circled in place and fell over, falling off the area he was standing and landing on his head and neck.

The employee was carrying a bag of rock dust on his shoulder when he stepped on an uneven section of mine floor. This caused him to fall and injure his knee. Employee was lifting a 5 gallon bucket of degreaser out of the back of a pickup truck and felt a pain in the abdomen area. Employee did not start missing time from this injury until 10/15/2015.

EE right foot became stuck between the longwall shield foot and the pan line relay bar. EE lost his balance and fell over, fracturing his lower right leg and dislocating his right ankle.

IE (Temporary Employee) was riding in a mantrip entering the mine. As the mantrip turned the corner at the bottom of the slope the IE's left foot was pinched between the steel plate on the arches and the frame of the mantrip. He received a contusion of his left foot and was taken off work.

Employee and 3 other men were picking up and pulling the miner electrical cable. Employee states ""I bent over and it felt like a knife went into my back"". Employee went down to his hands and knees; and complained of severe lower back pain. He was taken to the hospital ER and was discharged with a BACK STRAIN.

Claimed injury, sprain of lumbar spine ligaments, occurred while operating a front end loader at an unspecified date/time the week prior. During a return to work physical examination on 11/04/15, Miner described his injury had occurred while swinging a sledgehammer on 10/30/15.

He was released the night of 10/23/15

On 11/3/15 the mine operator was informed that the employee could no longer work at full duty due to a partial tear of the ACL in the left knee. On 10/22/15 the employee jumped from a ladder as the ladder was struck by a 28-inch ventilation tube elbow which was pushed into the ladder by a rib roll.

Employee was swinging a 20lb sledge hammer almost horizontally to drive a pin out of an excavator when he felt a sharp pain in his left upper arm/shoulder: was diagnosed in the ER as a bicep tendon tear.

The IE worked until 11-8 when the discomfort would not let him finish the shift.

Injured Employee was pulling a 1-1/2in x 15ft.cable,4ft. to attach to the back draw bar of his dozer that was stuck. As he attached the cable to the draw bar, he felt a sharp pain in his lower back.He continued operating the dozer until the end of the shift.

Skin not broken. No visible bruise. Clinic doctor limited the miner's mobility, pending an MRI to determine if undetected damage has occurred to the spine.

Employee was working on a Cushman haulster Cart with a flatbed that had flat tires. He was putting tools away and when he lifted the bottle jack up, he twisted at the same time he stood up and he heard a pop in his hip joint and had pain.

EE was holding onto the handhold swatting at the wasp when he lost his balance and fell ~6' to the ground suffering contusions to his hip and back and arm strain from holding onto the handhold. The post-accident drug test results revealed that the EE tested positive for THC and opiates.

Fall to the walkway or working surface

Smashed middle & ring finger tips on right hand while adjusting anvils on impact crusher. Lost balance while swinging hammer. I took him to hospital to clan & tape up fingers. No time was lost from work as he was back to work next day.

Struck against stationary object

He is 6 ft. 5 inches tall walking down a entry that is 5 ft. 6 inches tall. While walking he had his head tilted to one side, when he hit his ear on the end of a roof bolt. (he received stitches)

Night shift Maintenance Millwright was prepping a piece of flat bar steel 1/4""x4""x27"", using a bench disc sander. The steel was ""grabbed"" by the sanding disk and struck and punctured the hardhat, contacted the hardhat suspension inside, and continued downward striking the millwright just above the left eyebrow resulting in seven sutures.

Employee was struck by a foreign object while pulling conveyor belt in place.

Over-exertion in lifting objects

Installing 2' by 2' plate on raw coal screen and strained back when the plate slipped. The employee required physical therapy. This is being turned in as a medical treatment incident.

Struck by flying object

EE using an air hose to clean and got debris in his left eye. EE was given a prescription eye drop and released back to work with no restrictions.

Struck against stationary object

The employee was beating the cone at the bottom of tank #6 to loosen mud that had accumulated at the bottom of the tank. The head of the hammer glanced off the side of the tank. Because of this, he hit his left hand on the guard rail of the catwalk. This caused severe pain, numbness, and swelling in his left hand.

Struck by falling object

Employee was bolting on #1 unit in #2 left swinging over to put up his 2nd pin in his 2nd row when a rock slid off of the canopy and hit him in his lip and arm causing him to receive 4 stitches in his upper lip.

A can of starter fluid was tossed to employee. He missed catching it and it hit a motor, bounced off and stuck him above his left eye. He had 6 stitches above eyebrow to close up the cut.

Struck by falling object

Operator was bolting up 4540 heading. He went to repair a leaking hose when the right rib failed resulting in a buried bolter. He was able to escape over the top of the material.

Miner load powder to blast round. A slab was on collar pipe couldn't load hole without moving slab. Moved slab and broke free, sharp end cut through glove and cut right palm.

Struck by flying object

EE was working on feeder along with a fellow employee installing a new pit breaker chain. While trying to break the chain using a hammer and chisel, the other EE accidently let go of the chisel causing it to go airborne hitting the injured in the mouth. This caused a small cut to the lower lip and one (1) of his front teeth to break (chip).

Over-exertion (Not Elsewhere Classified)

Employee was attempting to install an hydraulic pump motor on a Cat scoop when he strained his right shoulder. It was determined on 10-22-15 that medical treatment would be required.

Employee was cutting conduit with a Metabo power tool, when tool kicked grazing the employee's knuckle. Employee had the wound cleaned and then 4 stitches were needed to close the wound.

Struck by falling object

Employee was using a hyd. jack with a 100lbs. metal spacer on top. When jack pressure was released the spacer fell hitting employees hand.

Struck by flying object

EE stated that while he was grinding on a weld Saturday he felt like dust in his eye but thought he wiped it out. He finished his shift and Sunday morning he woke and felt something in his eye. He went to the eye doctor on Monday morning where the dust was removed and he was prescribed antibiotic drops for his eyes. He returned to work the next shift with a full release.

Flash burns (welding)

Employee received flash burn to eye while welding.

Caught in, under or between a moving and a stationary object

Employee was in the shop area storing bucket pins on a storage rack. A pin fell over pinning his Right index finger between the pin and shelve causing the laceration of the finger.

Struck by falling object

Employee was rolling balls of aluminium foil to calibrate crusher, when the box of aluminium foil fell off the table. The employee went to catch the box, when the blade side of the box cut the employee on the hand. Employee was taken for medical care and received stitches to his hand.

Fall onto or against objects

Employee was walking and tripped on uneven ground causing him to fall and cut his right hand on rib. (Medical treatment)Sutures required. Laceration to right hand.

Struck by falling object

Employee felt an irritation in his eye after taking his glasses off during lunch break. He was treated at an optometrist for a mild corneal abrasion and given a prescription for an antibiotic making this a reportable event.

Fall from ladders

EE fell from extension ladder striking upper neck area on a 4"" protruding vertical pipe.
