Accident type, without injuries

The elevator went down at 9:55 PM, The elevator had just been checked by Lake Shore approximately an hour before. The problem was E-stop button on top of the elevator our guys changed the E-stop switch and it corrected the problem.

A roof fall above anchorage was found by the Mine Foreman in the 7 Seam A Entry return @ 36+75. This fall was approximately 60'x 17'x6.5. There was no injuries with this accident. The travelway was blocked but did not effect ventilation. The area was dangered off at all approaches and travelway was re-routed around the fall. This area was mine 9-1-2014.

At approx. 11:20 am the pre-heat elevator stopped with one miner inside. That miner was rescued unharmed.

A CM cut the fiber optic cable used for tracking. The dispatcher notified the RP and all personnel inby to begin manual tracking procedures. The repairman was summoned to make repairs. Approaching the 12hr limit mentioned in the ERP, the hotline was notified at 12:46pm Ref#00793039. No persons were injured or trapped. The cable was repaired and tracking restored at 3:00pm.

American Eagle Mine experienced an unplanned roof fall above the anchorage zone of the primary support at 14 brk in the number 3 entry (spad #22526)on the unit 4 section. The dimensions are 80' long x 20' wide and approximately 8' thick. No injuries are reported due to the fall.

At the west stock pile, 993K loader engine compartment caught fire which was eventually then fully engulfed the engine and cab of the loader. A mayday was called, mine rescue team was initiated. Additional resources were called out from lovelock fire department. 2 engines arrived on scene and the mines 20,000 gallon water truck extinguished the fire.

Packer partially deflated allowing methane to leak into the #3 entry for a short period of time. Methane accumulated in local area only, packer was sealed and methane cleared. No other 50.20 criteria applies. We dispute that this was an immediately reportable accident and information submitted is not an admission of any citation.

North loading gate was stuck open. It was found to have a piece of foreign material in it that blocked the gate from closing. The entire mine was Non-Emergency evacuated before the problem was found and corrected. All chutes, pockets, and screens, etc. were checked to be sure no additional material was loose.

The mine experienced a roof fall on the 3C Headgate, 18BLK, in the #1 entry. The fall occurred in an intersection and measured approx. 452 sq. ft. A pumper found the condition. There were no injuries from this incident.

An unplanned roof fall was discovered in the Conner's Run Return adjacent to the No. 1 Belt Drive. The fall is approximately 12' wide, 20' long and 5' high. Management is in the process of cleaning and supporting the area to restore safe passage.

Non-injury roof fall was found in 5 left panel off 4-North Mains. The roof fall was found during a weekly examination. Roof fall begins in No. 5 entry of 5 left panel and extends inby to crosscut No. 12. Roof fall then continued into No. 6 entry of 5 left panel. Roof fall appears to be approx. 4ft to 8 ft. thick and approx. 18 to 20 feet wide (see map). MSHA Hotline notified.

An alarm was sounded and confirmed that we had a fire inside our Coal Mill building. The fire department was called and found that C-720, WF-720 and WF-721 were on fire. They put the fire out, but not within 30 minutes.

The No. 77 mantrip was traveling into the mine when the brakes became hot and the caliper caught fire at 15-05 crosscut. The fire was reported to have burned for 10 to 12 minutes before it was successfully extinguished. No injuries occurred as a result of the fire.

An accidental roof fall occurred on the North Mains Belt between 35 xc and 36 xc. Upon investigation it was determined that the roof had fallen above the anchorage point at 4:27 pm. The fall was 8' wide by 20' long and 6' to 8' high. The area which was mined in 1994 fell due to deterioration.

Break 17 on 1 South Mains Return a roof fall was discovered in the examiners travelway. The area was dangered off by Jacks and beams.

a roof fall occurred at approx. 0240 on 10-24-17 on the 4th south panel in the intersection of 17 xc and 2 entry. The fall measured approx. 50' long by 18' wide ranging from 5' to 10' in height

Upon daily hoist inspection, the hoist computer system was malfunctioning which prevented the operation of the hoist. It was determined that the battery backup caused the problem. The battery backup was disconnected and hoist functions returned. The hoist was not repaired within 30 minutes.

An unplanned dome shaped roof fall occurred at or above the anchorage in the East Mains #1 belt entry at crosscut 49A. The fall measured 16 ft. long by 16 ft. wide by up to 6 ft. in height.

An unplanned roof fall was discovered in the Conner's Run return airway at the ""Blow Hole"". The fall was approximately 11' long, 10' wide and 3' high. It was temporarily blocking travel but has been cleaned and supported to reopen to safe travel.

An unplanned roof fall occurred in the 6L section in the #2 entry just out by 33 x-cut. The fall measured 15ft long by 6ft high by 10ft wide. The opinion formed during the investigation was that the fall was caused by adverse geological condition, in the form of a cutter running up the left bolt line.

Driven by high winds, a power line disconnect switch located at pole #80 on the N side of pit 15 - arced igniting dry grass. When the fire was discovered a 657 scraper, 16G blade and wildland truck contained and extinguished fire in 1 hour 10 minutes.

American Eagle Mine experienced an unplanned roof fall above the anchorage point of the primary support at 61 brk in the #3 entry on the old 2-South Mains track entry. The dimensions are approximately 20' long x 20' wide. All approaches have been breakered off and no escape ways were affected.

Roof Fall--70' in by the survey station #1042 in the cross cut between #4 entry and #3 entry on the 2nd Northwest Mains 3,400 feet from our section. This is an area where we pulled back from. It measured 6' thick by 16' wide by 35' in length. It is not affecting ventilation. We will not be cleaning the fall up, we are dangering it off. No employees or equipment was involved.

An unintentional roof fall occurred at cross cut 55, entries 1 and 2, in the tailgate #2. The fall in the number one entry was approximately 30 feet long by 20 feet tall, the fall in the number 2 entry took all of the intersection and crosscuts right and left for approximately 40 feet.

Unplanned roof fall @ xc #79 1/2, entry #1, 5-L beltline. Fall occurred in the pillar line between xc #79 and xc #80 area was supported with 7' TR bolts and 100 ton heizman jacks with cross bars. Fall dimensions - 30 ft. long, 19 ft. wide, 8 ft. high. Fall did not cause injury, inpair ventilation, or intrap miners. Fall area developed 02/11/2017 to 02/13/2017

Roof Fall in the 2nd north west mains, approximately 250 feet inby spad 1169 between a cribbed area and a solid coal rib. The fall is approximately 10 feet wide, 6 feet high and 30 feet long

There was an unplanned roof fall in the 1254 intake crosscut 156 entry #7.

2 roof falls discovered. 1st fall near spad 6577. 2nd fall near spads 6553, 6560 and toward spad 6573. No equipment or employees were involved. No ventilation controls affected. These areas have been timbered off and danger tags hung. Each fall was 19.5 feet in width and approximately 14 feet in height. A map of the falls were sent to the district office.

On 10-19-17 a roof fall occurred above the anchorage point inby the tailpiece on the 4WLW at survey station 100+95 in the belt entry. The fall occurred due to deterioration of the immediate mine roof possibly due to a streambed. Additional roof support will be installed to prevent recurrence. No other part 50 criteria applies.

An unplanned roof fall was discovered at 9:45 am in crosscut 14-35 between the track and belt entries. No ventilation or travel ways were affected. The area is being dangered off from travel.

Roof fall in 2nd North west mains approximately 325 feet inby spad 1168. Fall does not affect ventilation or passage of miners. The fall is approximately 18 feet wide, 7 feet high and 60 feet long and does take in an intersection that abuts a solid coal rib.

A motor lead for the east hoist motor was burnt in 2. A non-emergency evacuation of the underground employees was safely conducted. Repairs were made and the hoist system was fully tested.

Upon inspection it was determined that a 10 foot guide beam in the vertical service shaft was loose. The decision was made to completely replace the beam and the service shaft will be taken out of service for more than 30 minutes to complete this repair.

A fire occurred on the #11 hydraulic shovel when two gang members noticed sparks on the top of shovel house. Fire system deployed automatically and all employees on the shovel were able to safely exit the shovel. Smoke and flare-ups continued on the shovel until 12:53 when the fire department officially deemed the fire out. MSHA was notified at 1:41AM of the incident.

A roof fall occurred not at or above the anchorage point on #3 belt at 70 block which impeded the walkway on the right side of the belt. A rock approximately 7' long X 3' wide X 20"" thick was in the walkway. No other 50.20 criteria applies. We dispute the allegations in citation No.9125868. Information submitted is not an admission of any citation.

Shift supervisor noticed a small smoker in coal face while doing on shift inspection at 5:00am, when crew got to the coal pit the smoker had some small flames starting, small fire was due to spontaneous combustion of the coal. Overburden will be used to cover the fire.

Loss of control panel communication for the hoist operators control panel on the shaft hoist. It was found that 2 small breakers were knocked on a breaker box at the shaft collar.

Unplanned roof fall @ xc #6, entry #2, station #750, 7-L travelroad. Fall occurred in an intersection. 4-way, supported with 7' TR bolts, 12"" cable bolts, 100 ton heintzmann jacks + bars. Fall dimensions - 25 ft. long, 18 ft. wide, 12 ft. high. Fall did not cause injury, impair ventilation or entrap miners.

The mine experienced a roof fall on the 2 North Mains Section. The fall was located in the #1 entry, Last Open break intersection, at 23 Block. It carried approx. 6ft towards the #1 face and approx. 30ft into the 1-2 crosscut. The fall was found on an idle shift by an examiner. There were no injuries from this incident.

The 480 volt main feed power line going to the elevator had a phase burn into causing the elevator to stop. There was 1 miner on the elevator at the time of this accident. The line was repaired and the power was put back on at 12:00 pm 10/09/2017 and the miner was brought outside. There were no injuries with this accident. Schindler Elevator Company was called and on sight.

A non-injury roof fall occurred on the #3 section at approximately 22:00 on 10/9/17. The fall occurred in the neutral #3 entry in the intersection of Spad 8709 nine cross cuts outby the feeder line. The fall measured approximately 20' x 20' just above anchorage of 48"". There were no persons affected and the area has been timbered, cribbed and dangered off.

A roof fall was found around 1:47 p.m. in the track entry at 6 break along 3C belt in the intersection at which the actual time of the fall is unknown. The roof fall extended into the crosscut going towards the #3 entry. The fall was timbered and cribbed off and no injuries occurred. Both agencies were notified.

Maintenance mechanic was coming down the passenger elevator from the top of the 56 silos. The passenger elevator stopped during the flight to ground level. An electrician climbed the ladder way to the top of the silos, activated the elevator key to open and close the door and the car was recalled to the top.

A wild fire blew through the mine. The fire started off site and because of strong winds quickly engulfed areas such as the bone yard and timber by the rail. There were a few plant supplies and 80 plus ties lost due to the fire.

The conduit and control wiring had pulled loose from the Mine Level Door Switch. A short circuit was caused when the wires touched which in turn caused a PLC output card fault. The PLC was reset, conduit repaired and a new door switch was installed.

American Eagle Mine experienced a roof fall in the primary escape way for the unit 3, unit 4, and the long wall. The fall occurred in between 78 break(spad#12559) and 79 break(spad#12568)in the #6 entry on the old 2-South mains.

Pit supervisor was pushing dirt, when backing up the operator lost track of where operator was at and backed over a berm causing the Caterpillar D6H Dozer to roll backwards 270 degrees landing vertical on it's blade. The operator was able to exit the machine without any injuries. The dozer caught on fire, the fire department was call and the fire was extinguished.

A SMUD transformer exploded causing a grass fire near the mine site in the Aerojet easement. The fire was never actually on the mine site but called in for reporting purposes.

2 roof falls were discovered by the weekly examiner between spad locations 5918 and 5916, also from spad 6254 to 6256 on the old No. 10 belt panel. All approaches to the falls have been timbered off and danger signs hung. No employees or equipment were involved. No ventilation controls affected.

A non-injury roof fall was discovered in the No. 2 entry at X-cut #18 on 2 South Mains on 10/3/2017. Upon discovery of the roof fall the MSHA Hotline was immediately notified.

A coal smoker in the East Porcupine pit that was not extinguished within 30 minutes of discovery.

Haul truck struck man van resulting in two fatalities. A passenger van carrying nine miners was run over by a 340 ton haul truck.

Investigation indicates that the employee was attempting to cross a moving belt or cross a belt that had just started, when employee was pulled down the belt line and later recovered at the raw coal stockpile at the plant.

Caught in, under or between a moving and a stationary object

Operator was up on hill pushing overburden down to excavator for feeding crusher. Operator went down a small incline on Cat Dozer and was thrown from the cab.

Caught in, under or between running or meshing objects

Assumption is that EE lifted latches that secure a door, opened the door & put left hand inside mill (mill 02) while the mill was in operation. As of the date of this report the EE has declined to provide a statement of the incident & there were no witnesses. Employee suffered amputation to left thumb and 2 or more fingers.

Over-exertion in lifting objects

Employee felt pain in abdomen while lifting high voltage cable. Injury caused by employee overexerting self and not lifting properly. No mining equipment was involved and employee has proper training and was wearing required PPE. Received information on 11/13/17 that employee has been diagnosed with a hernia. Employee has not missed any work due to injury at this time.

Over-exertion in lifting objects

While removing an electric motor to be repaired the employee felt a ""Pull"" in the stomach area. EE was sent to the local medical facility and diagnosed with a hernia.

Caught in, under or between running or meshing objects

EE acted against policy/training by leaning a ladder against running screen box to clean the screens. EE began to descend ladder when it began to slide. EE reached out with left hand to steady ladder, contacting flywheel of screen box, resulting in injury. EE returned to full duty on 10/30/17, retrained on proper way to clean screens, fall protection and lock out/tag out procedure.

Caught in, under or between running or meshing objects

A Plant Operator removed a rotary valve cover to inspect for clay buildup. The Operator failed to isolate and lockout power to the R.V. prior to removing the cover, per company rules. The Operator placed left hand in the chute to remove some clay, and ee middle and ring fingers came in contact with the R.V. blades, amputating the distal phelanges on both fingers.

Struck by falling object

EE was observing the installation of roof bolts with Fletcher CB on the 4WTG section. A rock approx 4'x6'x12' fell striking ATRS then ee on the lt lower leg resulting in a comp fracture. On 10/8/17 the injury resulted in amputation of the leg at the knee. Cause of injury was awareness. Compliance with rules/regs,PPE,mining equip or Miner proficiency not a factor at this time

Employee was shoveling on 3A belt when they felt pain in the lower back, right side.

Caught in, under or between a moving and a stationary object

Haul truck struck passenger van resulting in fatalities. Individual admitted self to hospital for further post traumatic evaluation.

Struck by falling object

Carrying belt structure off the 1 East Headgate longwall belt and carrying it to 33 block to load onto a scoop. Injured dropped belt structure on left foot resulting in pain and a contusion.

Struck by flying object

WORKER WAS CHIPPING AWAY A PIECE OF STONE WHEN A PIECE OF THE HAMMER BROKE OFF AND RICOCHETED BACK OFF THE ROCK FACE STRIKING SUBJECT WORKER IN THE NOSE.

Struck by falling object

Employee was hit in the neck and shoulder with a cable while tramming the roof bolter on 10/31/17. EE continued to work until 12/15/17. It was not until work comp. reviewed the case and received the Dr's report that this injury was due to the incident that occurred on 10/31/17. We were notified on 1/25/18 of this diagnosis.

Employee was straitening bolt back to install in roof when they felt a strain in the left shoulder and elbow.

Haul truck struck man van resulting in fractured ankle.

Struck against stationary object

Employee was shoveling under belt and hit solid rock and injured finger.

Struck by falling object

Employee was loading shuttle car when ee completed the car ee turned to back the miner up to set over and a piece of draw rock fell and struck right hand resulting in a fracture. Employee continued to work up until 11/9/2017.

Fall from machine

Employee was exiting loader and fell off bottom rung of ladder and hit left elbow on ground. Elbow is swollen.

Over-exertion (Not Elsewhere Classified)

The employee had finished repairs to the Preblend Stammler system immediately prior to the injury, which occurred when employee was walking down the stairs with tools/equipment. The employee stepped down from the top step with employee's right leg, and as soon as employee put pressure on employee's leg employee felt employee's knee ""pop"".

Struck by falling object

A fully trained miner was unloading jacks from a mantrip when a jack was dropped and landed on miners right foot. The incident was not the result of any compliance issues nor were any protective guards involved.

Struck by flying object

While grinding on a guard that the employee was making for the 5' Pioneer screen employee received a piece of metal in left eye. Employee did not inform us of accident until 11/1/17.

Over-exertion in pulling or pushing objects

Employee was pulling manifold line from pumping and felt pain in ee stomach area.

Over-exertion in lifting objects

Employee strained back while shoveling a pile of loose material into the bucket of a skid loader in front of FM3

Employee was cutting face chain on the 4WLW face at approx 3 shield when the chain let loose pulling ee into the top deck of the panline causing lacerations to ee's head, a fracture to ee's nasal cavity and burns to ee's right arm. It was determined the chain was still tensioned and procedures have been put in place to prevent re-occurance.

Employee stepped on a rock and twisted knee.

Struck by falling object

Employee stated that ee was making faces. EE was in the #3 entry face dating up. As ee was walking out a piece of rock fell from the mine roof striking ee in back and head.

Operating miner when a large piece of draw rock fell from mine roof pinning employee to the floor. Resulting in fractures to back, nose and ribs. Lacerations to face and lip. Was hospitalized and later released.

Ee was mounting tractor, reached for handrail and missed, causing ee to miss the step and fall. EE has a fractured left wrist and right thumb. Seen at ER, put off work and referred to ortho.

A fully trained miner strained miners back when pulling a cart off of the Fishcreek elevator. The incident was not the result of any compliance issues nor were any protective guards involved.

Struck by falling object

Employee was working on miner in face when a rock fell on ee back pushing ee down into the 14/15 Joy miner, fracturing both collar bones and ribs on both sides. EE was hospitalized and received surgery.

EE was trying to lift mast on track tamper when EE slipped in wet gravel and strained EE's left knee. EE worked till 12/01/2017 when EE was taken off work.

Over-exertion in pulling or pushing objects

The employee was throwing a track switch on Pleasant Grove bottom when EE felt pain in right shoulder. EE was later diagnosed with a partial tear of the right rotator cuff and began missing work on 10/30/2017.

Fall from machine

EE was unhooking the bolts off the VSI and when EE was getting down instead of using a ladder like EE was asked to use twice EE just jumped down a 2 foot drop.

Over-exertion in pulling or pushing objects

Employee was repairing exhaust on a D10T dozer and strained their right wrist. Physician released employee to work. Physician prescribed a support brace on 11-22-2017 and the wrist required surgery on 1-16-2018.

Struck against a moving object

EE was fueling an excavator using a tractor w/a fuel cell on it. When done the operator started the excavator and began to spin around and hit the fuel cell on the tractor. EE said EE's back hurt a little so I asked if EE needed a Dr EE said no and finished out the day. Mon EE left 12:24 Dr. apt. Tues 10/31 EE had a follow up will return to work on 11/3

The employee was taking a scoop load of dust to the rooms when a ram car came through a pull curtain and hit the scoop.

Employee was operating a shuttle car, while tramming they encountered uneven bottom floor, bouncing them in the seat and causing pain to their neck. EE continued working without restrictions performing normal occupation until taken off work on 03/20/2019.

Over-exertion in lifting objects

EE was lifting a ventilation tube when EE felt pain in left shoulder. The EE continues to work but has started PT as of 11/28/2017. EE is now a NDL accident.

Bodily reaction, (Not Elsewhere Classified)

EE was on the L/W face pulling shields, a rock fell from between the shields striking employee in the right cheek, EE abruptly moved head to avoid rock and strained neck. Cause of accident was lack of concentration. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency was a factor.

Fall to the walkway or working surface

Walking in Jamaica building. While walking fell in hole. Injury left knee.

A fully trained miner Strained miners back when jacking rail up. The jack let loose while jacking causing the strain. The incident was not the result of any compliance issues nor were any protective guards involved.

Struck against stationary object

Employee got a small cut on the back of their hand when they bumped the roof while setting a bucket of bolter bits on top of the bolter. Employee did not get any medical treatment for the injury. Then on November 1, employee's hand was very swollen so employee was given antibiotics via IV to treat infection.

Employee was fitting a door on deck of Fairchild battery scoop deck & door fell off deck landing on left foot behind steel toe.

Fall from machine

While delivering material I realized that my truck was having issues. I called towing company who came & loaded the truck. Then took the truck to the quarry to unload the material. After unloading the material, I proceeded to step out of the truck only to discover that the wrecker had raised the truck further from the ground causing me to fall & injure myself.

While working to remove the lower corner liner of a mill, a corner liner above slid down impacting employees fingers.

Employee stated that they were taking the scrubber motor ductwork off the miner when it mashed a finger. They received 10 stitches to their ring finger.

Struck against a moving object

According to driver, got loaded in the pit and started pulling ramp up out of pit got about halfway up the ramp and broke traction and started sliding backwards and went sideways and upset. It had been raining off and on all morning

Operator injured lower back while pulling core from drill casing.

Over-exertion in lifting objects

Employee claims to have strained employee's lower back while shoveling spillage on the first floor of the prep plant. Employee continued to work and then called off on 11/06/2017 citing the earlier ""injury"".

Employee claims to have strained right hand while shoveling on the 1 East Belt. EE continued to work regular shifts. On 10/30/17, ee requested supervisor to document the incident of 10/24/17 as hand continued to hurt. EE started missing work on 10/31/17.

EE was attempting to lower 4"" HDP water pipe from the mine roof to the floor. The EE and another were standing the bed of a pickup truck. EE had the pipe resting on their shoulder as the other cut a nylon strap used to secure the pipe to the roof. The pipe shifted causing the EE to fall from the truck. The injuries included: Fractured rib and stitches above the eye

Operator was exchanging a motor 40lbs. and gear box 50lbs. when they lost grip, lost balance and the item fell and landed on their left hand. Basic first aide was given, employee requested further evaluation. Operator was transported to the hospital at 8:00P.M. Left hand was bandaged. Operator has a fracture and surgery was performed on 10/24/17.

Two employees were dismantling an elevated section of pipe from a catwalk. An employee was removing bolts from the pipe flange while the other held the other end up with one hand to alleviate pressure from the remaining bolts. When the second to last bolt was removed, the pipe swung down and pinched the employees left thumb between the flange and hand rail. 10 stitches.

We feel that this is a questionable claim however the employee stated that while pulling on an air hose EE felt pain in back. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.

Employee stated ee was hauling out of #6 entry when ee ran over a rock that had fallen in the roadway from between bolts. Employee stated ee jammed shoulders and neck when ee came in contact with the canopy. Employee sought medical attention on 10/23/2017 and was placed off of work.

Employee changing roller on head drive. Taking bull gear off, hitting bull gear with a sledge hammer. Hammer handle hit employee's safety glasses knocking them off employee's face. The head of the hammer hit the edge of the bull gear and a piece of metal went into employee's left eyeball. Employee had surgery on eye the next day

The driver was pulling a load from the mill location in the afternoon of 10/19/17. EE had an incident with the truck and trailer causing the load to shift. While trying to remove the straps from the load, EE fell off the trailer. EE broke wrist and toe. EE received stitches near eye, as well.

Process operator cleaned off level transmitter for RIL feed hopper. EE noticed the level in the vessel rising and exited the platform using the access ladder. EE made it to the ground and the vessel overflowed, causing hot slurry to hit them on the right side of the body, partially filling their right and left boots with hot slurry. The hot slurry burned their bilateral lower legs.

While changing belts on the jaw, an employee was using a cable come-along. EE was pulling the motor tighter to the wheel, the come-along slipped and employee rolled off 5ft platform. The employee fell on shoulder and side of face sustaining laceration and broken nose. The employee was wearing hard hat, but was not wearing high fall gear.

Employee was assisting a co-worker lift a de-watering pump (ABS 50) that was stuck and needed unclogged. As they were lifting, employee felt a sharp pain in the left shoulder.

While swinging a sledge hammer to un choke a rock chute the worker hurt left shoulder on 10/18/2017. On 10/30/17 went to doctor. On 12/4/17 started therapy. On 1/30/2018 first day lost to have surgery.

Employee was spooling cable onto cable reel, cable moved upward causing pain in right shoulder. MRI revealed: chronic avulsion, prominent tendonosis, severe degenerative arthrosis, and mild osteoarthritis of right shoulder.

The employee was pushing bags of lime briquettes into furnace retorts using an ~ 20' pushing rod. The rod caught on the inside of the retort and stopped suddenly causing a sprain/strain injury to the employee's right shoulder.

EE was stepping up from a feeder hole into a crosscut in order to put up a shuttle car anchor (3rd Shift). When put pressure on the left foot, ee felt a pop in left calf muscle. It began to swell and ee could not put weight on it. It was later determined at the hospital that ee had a tear in the Achilles Tendon and damage to the calf muscle.

While using a cutting torch to cut off mounting brackets for hydraulic lines on a Hitachi shovel hyd. fluid sprayed out and caught employees clothes on fire.

Employee was cutting #6 entry when a rock measuring 12"" wide X 30"" long X 1/4"" to 1.5"" thick broke from the top striking employee on the head and left shoulder area. Employee said neck and shoulders were hurting so after injuries were assessed employee was taken outside and transported to hospitals.

IE and another EE were in the process of setting the crusher in place. The overhead diverter gate lever was in their walkway; so EE raised the lever back with a 3/8"" rope that was looped through an eye at the end of the lever. When EE raised the lever, the rope broke causing the lever to swing free and strike IE's hardhat on the R side of IE's head knocking IE back into hand rail.

Employee was walking through a set of airlock doors when EE tripped and fell, striking EE's left knee on the mine floor on 10/16/2017. The employee returned to work on EE's next regularly scheduled shift and continued to work until EE was placed off of work due to the injury from the fall on 11/01/2017.

EE was roof bolting when drill steel broke off in the roof. EE went to get the broken part out of the chuck when the other half fell out of the roof striking pinky finger.

Began missing time on 11-08-17.

Grader operator entered cab of grader, sat down in the seat and began to take their jacket off when they felt discomfort in their back. The operator was seen by a medical provider the following day and given work restrictions which could not be reasonably accommodated.

Employee requested to be seen by an ERT member, initially declined further treatment. Later they requested to be taken to the ER. Employee was given 2 days off work.

Employee was picking up a stage loader flight out of a scoop bucket when EE slipped and smashed right ring and little finger between the flight and scoop bucket. This resulted in a fracture to the tip of the right ring finger and sutures in both the right ring and little finger. Employee is awaiting to see a specialist.

While changing a gear case on the continuous miner the pin cap became hung. A belt hanger was welded to the cap and a chain ratchet was being utilized in an effort to remove the pin cap. After applying tension the belt hanger separated from the pin cap striking the injured in the face.

EE was loading rock onto belt for splitting. Another rock fell on EE's finger. EE's right hand was caught between the rock EE was holding and the rock that fell. The 4th finger (ring finger) on right hand was broken.

Slide gate support failed and liquor sprayed on the face of mechanic. Mechanic was working on a valve and the slide gate support failed. Liquor sprayed on the face of employee.

Employee was riding out of the mine on a section portal bus. As the bus traveled through a switch, the bus was jarred from side to side. The jarring motion allegedly caused a sprain/strain to the employee's lower left back. Employee completed shift and went to a doctor later that day. Employee has not returned to work.

Employee was helping remove old electric motor on 2nd floor of plant when employee felt pain in left groin area resulting in hernia. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude.

Employee had gone to 3801 belt tunnel to winterize heaters. Employee was servicing south heater from an eight foot ladder and somehow fell from ladder. The injured employee was discovered unconscious by electrical maintenance worker that was looking for injured employee.

Working on D11R Dozer E-Bar center pin repair with customers job mechanic. Pin was frozen to correct temperature and both were holding each side of the 180 lb. pin when customers job mechanic stumbled. EE took the whole weight of the pin to keep it from falling on self. Felt burning sensation in stomach.

A hoist mechanic removing a check valve nut using a 4lb hammer and a pin hammer, when ee felt a pull in arm. The employee was treated on site, before being taken to the local hospital, and was released back to work pending further evaluation. Upon medical recommendation on October 26, 2017, employee elected surgery, scheduled for October 30, 2017.

The employee was using a 8"" hand held grinder to square up the excavator bucket pin boss face. When grinding employee got a piece of steel in left eye. Employee received medical treatment to have it removed.

Employee was stacking cement blocks on a pallet when ee felt back give out and ee dropped a block on upper left thigh. Employee was transported to a local hospital where all tests were negative and ee was released. Employee has not returned to work.

Employee was replacing a damaged hydraulic hose on a Komatsu WA380 end loader. A protruding wire on the hose punctured the index finger of right hand. The next day hand was swollen & painful. EE then reported the injury to the superintendent & sought medical attention.

IE was changing a bit on new drill steel at the start of shift. As IE was standing next to the bolter a rock fell from between roof bolts measuring approximately 4'X3'X4' - striking a glancing blow to left shoulder. IE was taken to the local ER for examination and was taken off work pending a follow up visit on 10/16/2017.

The employee lifted the tailgate to allow the material to fall and the tailgate to close. Operating walk through bolter, installing 5' bolt to secure an already installed pull plate in a high area in St Leo. The bolt started to bend with 8""-12"" left to go in the hole, so EE started to rotate it. The up pressure, rotation and angle of bend threw the bolt wrench out and struck the left side of EE's jaw, resulting in contusions and a laceration that required 1 suture.

The employee exited the dozer to look over the berm where EE was working. While attempting to walk up the berm, the employee reports EE lost footing and stumbled backward. EE reports that EE fell against the dozer blade. This employee complained of pain in hip and back. The EMT's on scene placed the patient on a backboard and EE was transported to the Hospital by ambulance.

A fully trained miner was injured when pulling a shield on the longwall face. A piece of dirt got in eye while operating the shield. The accident was not the result of any compliance issues nor were any protective guards involved.

EE was standing by some pallets, moving a tarp when another ee was bringing pallets over. ee did not move and the other ee had pallets in front of self. EE did not see ee and ee did not move out of the way. Other EE hit ee ankle with tire.

When the IE pulled the gate, it came all the way out and landed on IE's right foot causing a laceration to little toe requiring stitches. The IE was wearing all proper PPE.

Ee was operating a bolter. A/C unit not operating so ee had it fixed. EE felt nauseous with headache and tingling fingers. Taken to surface and given oxygen. CO finger reading higher than normal. Ee returned to work after O2 given. Seen later that day at clinic, where EE had denied ongoing symptoms. Released to full duty. Subsequent visit resulted in ee off work pending testing.

Operator was placing an empty super sack on a forklift. During the act of pulling the loops in place employee felt a sharp pain in the right shoulder. Dr. determined the injury to be rotator cuff strain.

Employee stated that on 10/2/2017 EE was attempting to lift a 5 gallon can of hydraulic oil when EE felt a pain in the back. Employee was coming down a set of steps just outside. As EE took last step with EE's right foot to the ground, EE heard and felt a pop just above the knee. EE fell to the right side on the ground. An ambulance was notified and employee was taken to Longview Regional Medical Center where the mechanic was diagnosed of having EE's right quadricep tendon ruptured.

EE was riding as the passenger on a golf cart. EE's left foot was under the right leg and when EE's toe caught on a piece of loose coal, the left foot pulled back, causing a strain. EE received a full release and continued working performing normal occupation until the surgery date of 01/03/2019.

EE was seen on 10/25/2017, and was diagnosed with Rotator cuff tendonitis.

The EE felt a lot of pain, went to the doctor, but had no fractured bones. Doctor restricted duty for 4 working days, thus making the accident reportable.

The employee stated ee was hanging tracking cable and stepped on some uneven bottom and twisted left knee. Employee finished shift and the next day went to med express and was diagnosed with a strained knee and was placed on restricted duty. The incident is being reported as a restricted duty.

An employee was removing chute door. When they removed last bolt, door fell and rolled over the foot of another employee that was observing the work. Employee worked the rest of the day then stated the foot hurt. Employee was sent to clinic and diagnosed with a bruised great toe. While backing a 775 CAT haul truck to dump material the rear wheels slide sideways into a slight depression and slowly tipped onto its right side. The driver was wearing seatbelt, was assisted out of the cab by a coworker, but bruised inner left thigh during the incident and was place on light duty.

Injured employee stated that employee was working on the conveyor chain for #302 shuttle car. Employee stated employee was on knees attempting to pry on the ball valve of the shuttle car so employee could add oil. Mechanic was using 2 lb hammer to drive pin into valve while performing maintenance activities. As the hammer struck the pin, the employees 5th finger (pinky) struck metal valve mashing finger between hammer handle and valve. The employee received 7 stitches in 5th finger. Restricted duty only, no lost days.

While the operator was ripping with a track dozer the operator states experienced some back pain while performing this task. Operator did not want to be seen by EMT and continued working. Employee still had back pain 3 days later and was seen by NP. Diagnosed with back strain. Treated conservatively.

On 15 Oct 17, employee was tasked to replace a light bulb. While doing so, the heavy light cover came off and either struck or strained a finger during the attempt to stop the cover from falling. Initially they thought the finger was sprained, but a follow up doctor's visit on 17 Oct 17 revealed a broken ring finger.

While attempting to adjust the forks on a telehandler, employee pinched thumb between the back of the fork and the backrest on the mast. The pinch caused the skin to rupture. Employee was taken to Emergency Room where ee received 6 stitches and was placed on restriction.

EE was in the process of hanging roof bolter cable. As EE started hanging the cable, the cable reel was engaged. The cable struck EE in the legs knocking EE to the ground. This was not reportable until EE had an MRI on 12/12/2017 and was diagnosed with a fracture of the medial tibial plateau and a meniscus tear.

When EE turned to retrieve supplies, EE's foot was caught in some mud and EE's knee ""twisted"" and popped causing the employee fell to the ground. EE was taken to the local ER, treated for an apparent patellar dislocation, given a splint, released w/out restrictions and is to follow up with personal physician.

Two people moving a gear box upstairs on a two wheel dolly. While moving it they had to turn a corner at the top of the stairs. EE felt soreness in the left hip & buttock area but it didn't hurt bad so continued to work. Then on 10/13 while working on brake lines under a pickup truck, EE was twisting and re-aggravated it. Rested for three days but saw a Dr and was put on RD.

While an ee was using a hammer & chisel to break chain apart another ee was observing. A small sliver of metal hit that ee in left wrist.

Installing D20T equalizer bar end pins with a 16 lb sledgehammer strained right shoulder. Did not seek medical treatment until 10/23/17

A Drill Helper had picked up a Full Core Box (Approx. 35-45lbs) from the Core table that it used to load the Core. When ee picked up the box and turned to place it in the storage location a few feet away, ee felt pain in lower back.

Employee was walking into work from parking lot and tripped on uneven pavement, falling to knees, bracing with left arm. Employee iced knees and shoulder for a couple of weeks. On 11/10/17, EE went to Urgent Care Clinic, was referred for therapy on shoulder and retractions. Notified company 11/13/17. Currently ongoing.

Employee was using a box cutter knife and placed it on a pallet of sand knee high with the blade exposed. The employee walked away and as ee walked back toward the pallet ee ran knee into the knife causing a laceration of the left knee.

Miner and partner were changing a feed cable on a CMAC DHS drill and while lowering the feed beam to provide easier access to complete the job the cradle that attaches the drill to the feed beam fell off of and landed on the edge of the working deck striking an object that flipped up and struck the employee on the cheek.

While replacing a blower motor on a Komatsu D65 Dozer in the repair shop, employee was standing on the right track of the dozer. The access panel of the floor board was on the dozer deck leaning on the open cab door. The access panel slid off the deck hitting employees right outside work boot.

Employee was installing a screw in an electrical panel. The screw contacted a power cable in the panel and created an arc flash which burned employee's left thumb and forefinger.

Struck by falling object

While repairing the support brackets for the utility body underneath a service truck, the employee attempted to remove the bolts from the bracket. Dirt and rust fell from underneath the truck and struck the employee on the forehead and fell underneath safety glasses. Dirt or rust went into the employee's eye.

Working on a wheel, hit ring with hammer and knocked it off and EE tried to grab it and it cut EE's right pinkie finger.

Fall to the walkway or working surface

An Administrative Assistant was walking back up the driveway, returning from the mailbox, carrying the mail in their left arm. After crossing the highway, they stepped on a small rock. It caused them to lose their balance and fall towards the right side. They tried to catch themselves with the right hand. Both the right hand and elbow landed on the concrete.

Struck by flying object

EE was lifting the rear end of an EMU with a come along when the hook slipped off and struck EE's top front teeth causing a chipped tooth. The EE had dental work done to cap the teeth on 12/01/2017 and this is now a reportable NDL accident.

Struck against stationary object

The employee was removing the tin end of a condenser containing a calcium crown when employee's tooling slipped and employee hit employee's left forearm against the adjacent crown causing a laceration. The employee was not wearing forearm cut protection as required by standard work.

Caught in, under or between collapsing material or buildings

Miners were working to clear the jammed primary gyratory crusher. 2 miners were working to remove stone from the bowl, one miner was working farther down in the bowl using an air hammer to break up a large rock when suddenly another rock became dislodged and pinned the miner's feet down, several rocks and finer material slid down as well entrapping the employee below the knees.

Employee was hanging a ventilation tube & a couple small stones fell from the mine roof. As employee stepped back to look at the roof a larger stone fell & struck employee on the top of the left foot.

Employee reported ee place hand on the canopy of the S/C when a piece of rock fell striking the employee on the left ring finger causing a laceration. Employee sought medical attention after shift where ee received two stitches. This incident being reported as medical reportable due to getting sutures.

Struck by flying object

Employee was grinding with hand held grinder wearing safety glasses and face shield. The next day employee's eye felt irritated employee went to the clinic and they removed a small piece of metal, employee received ointment prescription for eye. Employee did not miss any time.

Fall from machine

Employee was standing on frame of Trommell Power Screen prying on a belt scraper to adjust it. The bar slipped and hit the employee in the face causing them to fall to the ground, where they sustained injuries.

Over-exertion in lifting objects

Employee bent over to pick up a welding stinger weighing less than 5 pounds when they felt a sudden and sharp pain in the lower right back area. Employee was unable to put any weight on their legs. Resulting diagnosis was Sciatica.

IE was walking in muddy area and tripped over hog panels that were lying in the mud. IE fell on side straining back and hurting ribs.

Struck by falling object

While performing maintenance on plant grizzly hopper the miner went to tighten the bolts underneath. Miner's fellow coworkers were working above miner and when they went to move the light and tools the hammer fell between the grizzly bars. The miner looked up placing arm across to shield face. The hammer hit miner's arm and bounced off, hitting miner in the mouth.

Employee pinched right index finger between the gate and the roller while closing the gate causing a laceration that needed two stitches to close wound.

EE was riding on a 2 man rubber tired Johnson and ran over some gob in the roadway causing ee to jam head into a roof strap. EE felt pain in neck and shoulders and into both hands.

EE was demolishing brick and mortar from a tank with Rivetbuster. The bit broke on the Rivetbuster and caught left index finger between the wall and the equipment

An employee was washing down a strainer basket when the stream of water and debris was redirected into the face of the operator.

Employee was removing metal racks from a trailer. They dropped metal rack to ground, it bounced back up and cut into their leg.

Employee was removing pins from a jack on header. Another employee struck pin from opposite side of header striking employee in left index finger causing a fracture and requiring 2 stitches.

Employee was dragging a roof bolter cable on right shoulder as EE was going through a set of fly pads. While using left arm to raise the fly pad EE struck a roof bolt plate with left elbow, causing a puncture and small laceration.

When opening up a radiator cap after letting the equipment cool, the cap shot off and fluid came out onto left forearm

Employee was stung by wasp while preparing for plant move. While employee was being treated at Urgent Care employee lost consciousness. Employee was released a few hours later after being treated at the hospital.

Employee was using a utility knife to cut a piece of belt material to replace a skirt on hopper feeder #6. The utility knife slipped causing a small laceration to employees left leg. Employee was taken to the clinic and received 4 stitches then returned to full duty on same day. EE received a full discharge from care on 10/25/17.

Struck by falling object

While operating the continuous miner a piece of draw rock fell from the roof, measuring 6"" wide X 6"" long X 1/2 inch thick. It struck the employee on the back of the neck. 4 sutures were used to close the wound.
