We lost the control input to the main breaker on the drive for the hoist. The breaker would not close when required.

EE was traveling in packhouse elevator when it stopped at 3:24am, EE notified Central Control at that time. EE exited the packhouse elevator at 5:25am. The cause of this incident was a door lock that was jammed in the closed position. This elevator last had maintenance performed on it on 10-27-20 by Thyssen Krupp.

Two miners were trapped on Miracle Run Elevator while riding down into the mine. The safety switch was tripped, and the miners were brought to the surface on inspection mode. Once the appropriate people arrived, the switch was reset. Miners were off the cage at 6:02 PM.

On 10/28/2020 service power (Cleco) was interrupted due to a tree branch falling on the powerline off site. The emergency generator was started to evacuate the mine. The transfer switch did not fully engage on the first attempt.

An unplanned, non-injury roof fall occurred in the #6 - #7 crosscut. The fall measured approx. 4 1/2' feet thick, 35' long and approx. 18' wide and no persons were injured or equipment damaged.

An unplanned roof fall was discovered in the Old 5 North Mains #1 Entry at 8 block. The fall is approximately 120' long x 10' high and 16' wide. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues. This fall also did not disturb any ventilation or escapeways.

An unintentional roof fall was found at this operation at 5:45PM in the CO#4 entry of the active retreating 001 MMU. A high angle slip fell in the cut and pulled 2 permanent supports. Rock slid out on the side of MRS#7 blocking it from tramming out. There were no injuries.

2 Hoist kicked mid shaft Electrical faults while running hoist checks. 2 hoist was running at 11:00 pm.

A fire at the tail of BC8 surface belt was found by surface employees. This fire was in the tunnel exiting the crusher building. Fire was out by 5:00 am. A distance of 135 feet of top belt and 135 feet of bottom belt was burned. All communication to and from the mine was lost. Was able to contact MSHA at 4:35 am by cell phone by leaving the mine property.

BO waste line in 2 shaft. Wet Electrical boxes. 2 shaft was fixed and running at 11:15 p.m. on 10/25/20.

A planned outage was took to replace the motor on the NP Hoist. The job was canceled after it began. The hoist was put back together and tested before being put back into service. MSHA was notifed when it was taken out of service and when it was put back in service.

At approximately 12:00 noon, the operator noticed flames in the engine compartment below the radiator. The operator shut dozer down and along with the foreman proceeded to extinguish the fire. Fire put out within 20 minutes. They went to get more fire extinguishers and upon returning approximately 20 minutes later, the dozer had reignited and was burning rapidly.

The SP Material Hoist was taken down to change an axel to some wear on the wheels. This was a planned outage and MSHA was notified when the hoist was taken out of service and when the hoist was put back into service.

No. 3 Hoist - pulling muck from 4300 pocket, hoist kicked out, would not reset, called Spencer Engineering with computer faults. Electricians called, found two bad electrical components, replaced and safety checks conducted on hoist.

A roof fall occurred on 10/22/2020 during the night time & no miners were in the mine at that time. This occurred at Panel #3 W at cross cut 38 F3 & 38 G3 in an area that had been mined. The area has been barricaded & a formal geotechnical engineering study of this area is bein conducted. It appears the fall occurred below the anchor zone of the 42"" fgr bolts.

An unplanned, non-injury roof fall occurred in the #1 pillar entry on the #1 section. The material fell onto the continuous miner and two mobile roof support systems and had to be removed to get the equipment out and back into service.

Unintentional fall above anchor point in 9 Butt near spad #4869. The fall measured approximately 18 ft by 95 ft by 6 ft. The fall occurred as a result of a cutter that formed in a geologic transition zone several weeks after development.

Found an unintentional roof fall in the intersection of No. 7 Entry, Break No. 33, on the 2nd Primary Mains.

Incident caused by an excess volume of natural gas that entered the kiln during the relighting process and then ignited in a delayed manner. This higher-than-normal volume of natural gas then ignited and caused a pressure wave to propagate through the kiln damaging the boiler housing and kiln firing hood.

10-19-20 at 3am the socks inside the #2 dryer baghouse caught fire, the fire moved to the outside of the baghouse. The fire department was called, the fire was extinguished within 35 mins. MSHA was immediately notified and was on site for the investigation on 10-20-20; the investigation is ongoing. We believe the socks caught fire while cleaning hot fly ash from the #2 furnace.

The brakes on the hoist car were not releasing. The CR relay was replaced and the brakes were then functioning correctly

There was communication loss between the top and bottom of the slope. A Circuit board was replaced in the PLC for the fiberoptic communication.

A roof fall occurred on the 3ME at 146 crosscut between entry 4-5. The fall was 40'L X 19'W X 4'H. Ventilation was not interrupted and no one was injured from the fall.

A Roof Fall occurred in 2nd West @ the Face-Intersection of XC #40 in Entry #4. It appears to be about 7' height, 19' wide and 35' long. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses is that vertical cracks in the strata, parallel with the crosscuts, allowed the strata to fail. New Primary & Supplemental Support has been installed.

74 HG 16 xc 2 to 3 entry partially fell in on 10/06/2020. The same xc fell in the rest of the way across the xc at 3:18 am on 10/15/2020. The area already had breaker rows installed to all approaches. No injuries occurred. Additional roof support was installed in the intersection in #2 entry to protect it, and also in 2 - 1 entry.

Airway person found the fall on EE's examination of the airway. It's on old 1B panel in #2 entry BK 53 1/2 39' outby spade #3654. 15' high and about 40' long. It was also bolted with 4' bolts.

While removing a 15ft section of old 6"" slick line in the Leeville #2 shaft, the pipe brackets below the 15ft section became loose and allowed the pipe to shift slightly from the shaft wall. The #2 shaft was shut down until the brackets and pipe could be secured back to the shaft wall

Person in the area said the top started dribbling and then the intersection fell in. Area that fell was 8 feet high, 45 feet long in the beltline and 15 feet into the crosscut.

An unintentional roof fall was discovered by a Mine Examiner that measured 18ft wide by 60ft long by 5ft high. No equipment or ventilation was damaged during the fall. The fall will be cleaned up. The location of the fall was on Track Mains Section in the #1 Entry @ #18-19 Block.

Found an unintentional roof fall between breaks no. 9 and no. 10 on the #3 Belt line. Fall dimensions were approximately 70' long by 18' wide by 10' thick.

Found an unintentional roof fall between break no. 22 to no. 23 on the # 3 Belt line. Fall dimensions were approximately 50' long by 10' wide by 8' thick.

An unplanned roof fall occurred at the intersection of crosscut 27 on the DS-5 Panel of the 005 MMU in the #4 intake entry. The fall was approximately 30' in length, 20' in width, and 8' to 10' in thickness.

Smoke was observed coming from the north end of the raw coal storage pad near the Prairie Eagle east portal. Steps were taken to treat and remove the affected material.

An unplanned roof fall measuring 50' long x 16' wide x 9' above normal mining height occurred at 128 x-cut in the 7 Left Section between the track and return entries.

At approximately 4:20pm, a roof fall was reported on the 8 South #3 Belt at 17 3/4 to 18 crosscut. The fall was approximately 40 feet long and 16 feet wide and 8 feet high. No other part 50 criteria applies.

No injuries to persons/no miners exposed. Incident occurred overnight, no one was underground. Accident investigation is ongoing to determine cause, including if there was a pillar failure. Area affected by the fall included NW corner of M29&S37 to the NE corner of M29&S49 to the SE corner of S49&M47 to the SW corner of M47&S37 and back N to M29&S37. No equipment involved.

A roof fall occurred at 16 xc 2 - 3 entry in the 74 HG section. The fall was 30 foot long, about 6' high and from the center of the entry to the in by rib. It fell at or slightly above the anchorage point. No one was injured and the area was secured with posts and cribs.

A reportable roof fall above the primary support anchorage was discovered at approx. 6pm on 10/5/2020 by an electrical supervisor troubleshooting a tracking problem on West Mains. The fall extends from the brattice wall on the roadway to the J-Mains belt discharge on West Mains 3 belt. MSHA was notified and inspector arrived at the mine site on 10/5 approx. 2300 hrs

HAD POWER TROUBLE, EVENING SHIFT FOREMAN CAME OUT TO CHECK PUMPS AND DISCOVERED ROOF FALL AT SPAD# 1969 (168 BREAK ON 7 BELT APPROX. 19 FOOT WIDE AND 10 FOOT HIGH AND 50 FEET LONG ON TOP OF HIGH VOLTAGE CABLE).

An unintentional roof fall was discovered on the Spring Fork Panel #8. The fall was found on the pre-shift exam at approx. 8:58 PM on 10/4/20. The fall was approx. 16' high, 65' wide and 25' long. There were no injuries.

Roof fall was found during belt preshift. It had only fell 3' high. Examiner reported it to shift coordinator. When coordinator arrived with timbers and workers, it had fell up to 8' high, making it reportable. Coordinator had it reported. Fall did not affect mining, ventilation or escapeway(s).

The axle on the Material Slope Car went down. The car was removed from service, the axle changed, tested, and put back into service. MSHA hotline was notified when the hoist was took out of service and when the hoist was put back into service.

Feather edge false roof fall resulting in a death

Caught in, under or between a moving and a stationary object

Task was to remove screen flare. Employee was loosening bolts, changed work location, dangerously close to excavator, without warning or eye contact with excavator operator. Excavator rotated, bucket of excavator struck employee in the upper chest resulting in death.

Employee was operating a pick-up truck on the -330 bench. A haul truck turned onto the -330 bench. The haul truck operator indicated they did not see the pick-up truck resulting in fatal injuries.

Struck by powered moving object

The victim was fatally injured when struck by a battery powered scoop which had trammed through a line curtain in the #6 Entry of the #5 Unit (MMU 009-0) outby the working place where the continuous mining machine was being operated.

Struck against stationary object

Employee was on Hydro seeder and changed barrels on the gun and pointed the gun at self and engaged the clutch. The pressure knocked employee back against the rails on the truck. CPR was provided but had no effect.

Caught in, under or between running or meshing objects

Maintenance work was done on conveyor belt. Crew had meeting prior to bump. Employees were instructed to stay in one spot away from the unguarded area when bump was going to be active. Conveyor belt guard was left off during this process. Injured employee walked back to this area and stuck arm back in when the bump was activated.

An inexperienced miner helping with dredge line repair. Employee #1 loosened the haul back. The cable unreeled very fast and cut off the tip of employee #2 middle finger on right hand.

Struck by falling object

EE was installing material while standing on a scaffolding plank at the bottom of the cone section of an enclosed vessel when debris from beyond the vessel where the employee was working fell and crashed thru the protective scaffolding deck striking EE and causing injury.

Caught in, under or between running or meshing objects

Baghouse rotary feeder was plugged. Employee was opening a slide gate. Employee had positioned a ladder for access. Employee stepped off of the ladder and on to the screw housing. Employee's left foot went through the screw housing, got caught in the screw. Their left leg was amputated below the knee.

Employee was walking down the steps from the slope belt henhouse when employee felt pain in left knee. Employee continued to work till 03/08/2021 when employee was taken off work for knee surgery.

Struck by falling object

Employee was taking drill steel out of the hole they drilled, rock fell and hit left hand pushing it against the drill pod. Causing laceration to left index finger.

A fully trained miner was standing under a hanging cable when the cable was pulled down and struck them in the nose. The accident was not the result of any compliance issues nor were any protective guards involved.

While employee was attempting to enter the deck of a scoop, the scoop rolled slightly injuring EE's ribs on EE's left side.

Fall down stairs

While performing morning routine housekeeping, employee went to exit the catwalk going down steps, EE slips on the first step. Employee was holding onto the handrail and hit knee on the steps.

Contact with hot objects or substances

Injured employee was in a boom lift using a cutting torch. The worker turned the torch off and draped it over the handrail. The acetylene valve knob bumped against the mid-rail of the basket opening the valve which allowed acetylene gas to penetrate the employee's pant leg. A spark from the grinder ignited the gas causing a flash fire and burn to the worker's left leg.

Over-exertion in pulling or pushing objects

Injured employee was dragging ventilation tubes on the 3W CM Section when EE felt a ""pop"" in the back of the right knee. Has pain in leg. Was later diagnosed with a pulled hamstring muscle. Employee didn't violate any rules or regulations. Was wearing required PPE and has proper training.

Struck by falling object

Employee alleges EE was reclining pallets on a metal column, a burst of wind pushed one of the pallets and it hit the back of employee's left calf causing a laceration that required stitches.

Over-exertion in wielding or throwing objects

Employee was using sledge hammer to unplug #1 ore bin and sprained EE's right shoulder. Employee went out 1/5/2021 for surgery on right shoulder. From our Independent Medical Evaluator report on 3/31/21, this injury is reportable.

Injured was walking around the feeder to meter a cable and hyper extended left knee. Injured continued the shift and then on 11/2/20 went for medical attention and was then placed on restrictions for left knee sprain.

Over-exertion in pulling or pushing objects

On September 23 2020 employee hurt shoulder when taking out a water line from a tank on the water car and continued to work to October 28 2020. On October 29 2020 employee is having surgery done on the shoulder.

Struck by falling object

Contract IE was getting ready to replace lagging on a belt head roller, the belt was raised to get head room to work, a bolt thru a chain broke allowing the belt on one side to fall, hitting Contract IE on hard hat, neck and shoulder. Cause of this injury was using an old existing chain for rigging. PPE/mining systems/compliance with regs not a factor.

Fall from machine

Employee was descending the small WA70 Loader and slipped due to wet conditions and fell to the ground. Employee started missing work 10/30/2020.

Over-exertion in lifting objects

IE was hanging miner cable and strained lower back. Employee's low back started hurting later in the shift while driving shuttle car.

Operator attempted to pull 50lb bag from top of completed stacked pallet. Bag fell from top jerking employee's arm down. Employee worked without restriction until 2 Feb when they went out of work for what was told was unrelated work injury. 8 Apr was notified that the injury was work related.

Adjusting seat in the loader.

Over-exertion in pulling or pushing objects

Employee was pushing a wheelbarrow and felt pain in lower back. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Contact with electrical current

Employee was inspecting a newly installed 13.8Kv. switch and came in contact with 1 Phase of 13.8Kv. Resulted in electric shock injuries and burns. Employee received medical care from Surface Rescue Team and Company Doctor. Employee was medi-evac to a Medical Center.

Fall to the walkway or working surface

Employee was going to grease a truck and had climbed down the ladder to the ground. EE had parked on a flat area near the fuel tanks. EE turned around after climbing down the ladder and tripped over some rocks, causing them to fall over backwards and they landed on left shoulder, causing a sprain.

Employee was shoveling the ribs in the 4th East Mains when a piece of coal fell from the rib and struck the employee on top of hard hat and left foot. Employee was diagnosed to have sustained contusion and neck strain.

Over-exertion in lifting objects

IE was building a brattice when they picked up a block. When they twisted to set the block in place they felt a pain in lower back.

Employee was repairing a door closure mechanism when struck in the head requiring 4 stiches.

Struck by falling object

Employee was changing bits when a piece of rock from the corner of the face hit employee's hand that was on the head of the shear.

Employee alleges EE was moving mill plates and EE's left index finger was pinched between 2 plates causing a bruise/hematoma. Ice pack was provided as first aid.

Struck by falling object

Employee thought that had lost suction to the drill head and EE placed hand on the drill chuck to check for suction. When EE placed hand over the chuck, EE's drill steel that was left hanging in the hole broke free and fell hitting EE's finger against the drill chuck. This resulted in a displaced fracture to EE's left ring finger.

Employee was operating shear when a rock came over the handrail striking employee's left lower leg resulting in fracture to fibula.

A fully trained miner was attempting to re-rail a car by using the jack walker of the motor and a chain. When the motor lifted the car the chain broke striking EE in the shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee states repetitive use injury severe low back pain. No specific incident at this time. Referred to Orthopedic Institute. EE placed on restricted duty and placed out of work beginning 10/28/20.

Struck by falling object

IE was throwing CM cable out of the way when a rock fell between the bolts and slid down EE's arm catching EE's thumb between the remote box and rock. A laceration was received on the thumb.

Contact with hot objects or substances

EE was using a cutting torch cutting a piece of steel. Appears that the oxygen line blew out causing severe burns to EE's right arm. ER treated EE for 1st and 2nd degree burns and released EE

Employee was in breakroom before EE's shift started and was opening a container with personal knife and accidentally lacerated EE's wrist.

Employee states that while riding outside on the #4 mantrip that the mantrip spread the track at bk7 on 9c. When the mantrip did this, the employee stated that EE's back hit the metal seat causing pain in the lower back and rib cage.

Employee was assigned to remove a ball stud on 785C rock truck. Employee was using a 20lb hammer to remove a pin. The employee struck self with the hammer.

Knocking the end off a piece of curb and it fell and landed on the crossover step and bounced and landed on EE's right foot

Unjamming Syntron and ripped off nail and skin almost to bone.

A piece of shale and coal fell from the longwall face bounced off of the chain and hit employee on the left foot.

Employee was doing general maintenance at the crusher, employee strained back installing flashing at cone crusher. Sought medical attention on own. Continued to work normal shifts. Was not reported until 12/29/20. Visited Dr. on 12/28/20. Could return to work on 12/30/20.

An employee was helping carry a submersible pump from a standing metal table to the bed of LV022 (Ford Truck). The employee felt a pain in EE's back. The employee stated the back did not start hurting until the pump was sat down and the pressure was relieved off EE's back.

A fully trained miner was unloading bed rail when a piece slipped out of hand landing on foot. The injury was not the result of any compliance issues nor were any protective guards involved.

Over-exertion in lifting objects

Employee was hanging up the miner cable when employee felt a pop in right shoulder. The employee continued to work until 11/9/2020 when the employee was taken off work for right shoulder injury.

Caught in, under or between a moving and a stationary object

Employee was attempting to remove a pin from a clevis that was attached to a continuous miner rub rail that was removed from the machine and lying on the concrete floor. When the pin was removed, the rub rail shifted, smashing the employee's left middle finger between the rub rail and floor. This resulted in an avulsion of the fingertip and a tuft fracture.

EE stepped in a mudhole while pulling leg out of the mud EE fell forward and overextended right knee. Employee did not miss work until 10/30/2020.

Employee was dragging driscoll line and walked through a hole filled with water and allegedly emulsion causing irritation and burning to the employees legs. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was tightening the antennae on a Komatsu 500 loader from the right side platform. EE lost EE's balance and fell onto the handrail striking EE's left rib cage. Employee suffered a broken rib.

4900-ER6 getting started bolting, tripped over bull hose landing on rock with right knee. Reported to supervisor on October 23rd, did not seek medical attention till November 9, 2020.

Employee was working on a pump when EE felt a strain in arm, EE didn't seek medical attention until 11/18/20 becoming a RLT. Cause of this incident was lack of ee awareness. PPE was not a factor, mining systems was not a factor, compliance with rules/regulations was not a factor, miner proficiency may have been a factor.

Employee was using a crane to move a welder. The welder hit a eyebeam which was propped up on a ledge causing it fall on employee's foot. Injuring foot.

Employee was in the process of positioning the roof bolting machine to install roof bolts in the first cut taken for the number four (4) to number five (5) crosscut when a small scale of roof rock dislodged from between installed roof bolts, striking Employee on top of head resulting in a neck injury.

Security Guard was checking the site on foot and tripped falling face first to the ground.

Tearing out structure in deep mud, and twisted knee. This resulted in a meniscus repair on 12/09/2020.

Employee notified the Safety Director on 10/22/2020 at 3:00 P.M. that employee received medical treatment for pain in right shoulder caused by constantly correcting the blade tilt control of the D11R dozer employee was operating. The pain got progressively worse even after the blade control was repaired.

Employee was raising the light tower with the crank. The wire snapped and hit employee in the face.

While seeding, the dozer blade caught the ground throwing the employee into the rollover protection structure, resulting in a head laceration.

A fully trained miner was walking to throw a switch when they slipped on a rock and twisted a knee. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was on 6 Section drilling a rib pin and was having trouble with suction and went to pull the steel out and lowered the pot and left hand index was caught between the mast and the pot of the roof bolter causing a severe fracture to the finger.

Miner was preforming a conveyor belt patch when EE's knife/razor blade slipped off and cut EE's left arm. Cut area was thumb/wrist area and miner was taken to hospital due to bleeding. There EE was bandaged, stitched and stapled and released at approximately 4:30 on that same day. Miner was instructed to come back for reevaluation on 10/28/2020.

An OSMI foreman was driving an electric locomotive into the mine and upon approaching a run around, the foreman reached down for a bottle of water. At the moment they sat up, the foreman fell from the locomotive and was crushed between the locomotive and rib. The foreman suffered broken ribs and a fractured pelvis.

EE had worked the hoist 2 cranks when the belt broke outby the belt clamps allowing the clamps to fly forward knocking 2 top rollers loose on the belt pinning lower left leg between the belt structure.

Employee was riding as a passenger in an off road dump truck, while traveling down the ramp towards the pit. The truck gained speed and the operator maneuvered the truck towards the berm. Once the truck made contact with the berm, the truck flipped onto its side.

Contract employee noticed smoke coming from haul unit. EE placed unit in park and exited cab to inspect engine. After noticing a fire in the engine compartment EE jumped over the rail of the 770 haul unit and fractured left leg when landing on the ground.

When the lead separated, the connector flew up and hit the employee in the mouth breaking two upper front teeth, and cutting their lip. EE removed a lifting hook from an autocrine and placed it on a round shaft. A few minutes later the EE slipped in the mud and hard hat fell off. EE bent over under the shaft to retrieve hard hat. Placed a hand on the shaft to balance themselves and knocked the lifting hook off. The hook fell onto the EE's head. EE received stitches. EE returned to work.

EE started pump at remote quarry location, turned, homeless man (been there before) was right there. Man grabbed/slammed EE to ground, ran away. EE's wrist injured as EE tried to break fall. Dr diagnosed as left wrist pain with restrictions still allowing EE to do normal duties. Follow up x-ray was taken, revealed small fracture/bone chip in wrist. The police were called.

While EE was performing work of replacing a hose on a piece of equipment and also driving a pin back in EE got material in eye causing irritation. After multiple flushes it was still irritating EE and EE was taken to the doctor to have it examined- no foreign body was currently found or abrasion. EE received drops for eye and was released to full duty.

Two mechanics were working to remove drill steel on a Sandvik Jumbo in surface shop. One mechanic struck the drill steel with a hammer to dislodge the steel from the striker bar. The steel came loose and struck the other mechanic on the left hand middle finger that was resting on the Jumbo feed rail.

EE was attempting to loosen brake cartridge with a wrench. When the cartridge broke loose, EE's hand contacted adjacent metal manifold causing a laceration. EE received 5 stitches.

Customer Truck Driver, Not a Miner or Contractor: A customer truck driver jumped from the top of the trailer to the ground in an attempt to confront an employee of the mine that was telling customer to get down off the top of the trailer. The driver admittedly knew it was not allowed to climb on the trailer but did so anyway.

At approximately 02:30AM a 19 year old trespassed onto Vulcan Quarry. Person climbed over two 6' fences to enter the property. Person fell from the top fence of the high wall to a quarry ""bench"" approximately 300' below. The body was recovered on Sunday October 11, 2020.

During a load out a non employee truck driver climbed on the rear of a skid steer forklift and broke left leg after being expelled from the equipment when the load shifted causing the skid steer to tip forward. An ambulance was called as well as MSHA Norman office and Poteau office.
