The crabapple stopped mid shaft with 2 person aboard. The employees were entrapped for 30 Min. No injury.

At 5:25pm a grass fire was spotted by a railroad employee at Tatum interchange near the new construction site. the fire was extinguished by the Beckville fire debarment It burned approximately one hour.A bird was found on a electric line and is believed to be the cause of the fire. Dry conditions enhanced the fire.No miners were involved in extinguishing the fire.

A grass fire was noticed behind the South Crusher Stockpile as an employee was passing by. The employee notified site management of the fire and called for a water truck to aid in extinguishing. A local volunteer fire department also arrived to give assistance. The fire burned for approximately 45 mins before being extinguished, and burned 3-5 acres of grass.

Date of fall is unknown, it was discovered on 7-30-13. 2nd NE Parallel between spads 5+60 and 6+30, xc# 8-9, entry # 6. Measuring aprox 20'L x 16'W x 6'H.

A Roof fall was discovered during the inspection period of the weekly run in the 4-West right return air course. The fall is located at the 25 block, intersection of # 8 entry. The fall is above the anchorage bolts, no person injured, ventilation not disrupted but the walkway is obstructed. Area dangered off and approaches have been supported with roof to ground support.

Mine Examiner found a fall at Break 16 in No. 9 Entry at spad no. 2858 on West Mains. Fall measured 18'x12'x6'. No travel blocked, no ventilation blocked, and no injury. Fall was dangered off.

A roof fall occurred at 10:05 AM at 10+50 entry 6 in 8LMS MMU 010. Fall was in a four way intersection that had been mined May 8, 2013. Fall was 18' wide X 18' long X 8' high. Fall was caused by a transition zone that went from 4' pins t o8"". No injury resulted.

Ground fall went beyond the longest support in stope 2900W 10200 East heading. MSHA was called & notified. They came out & did inspection/investigagtion. We have repaired with shot crete, bolts & wire. A brow has been established & the cut has advanced. We will continu to established mining procedures to safely mine the stope.

An unintentional roof fall occurred at 10:30 PM inby x-cut 21 in the Headgate #2 in the number 3 entry. It is 14 feet wide by 14 feet on the left side, it is about 9 feet high.

The crew leader reported that a mechanical scaler had a possible ignition in 156R heading. No one was injured, no flash was seen and no flyrock was observed. A hand scaler was sent into investigate and a spent blasting cap and some yellow cartridge paper was found in a hole. The hole was approximately 3"" inches deep. MSHA was contacted.

During the examination of a weekly aircourse a cont. of an old fall was found. The fall occurred in the Ellis Main at break 27 and now extends from #5 entry to #4 entry. The travelway was not affected.

Concrete rubble was observed on top of the service skip by DMC as they were preparing to begin the day's work in the shaft. Concrete from the shaft liner broke free and contacted the bonnet of the service hoist causing minimal damage.

Employee was loading rock trucks and as far as we can tell a Hyd. Line broke and shot oil over onto the turbo. A fire started which could not be put out in the 30 minute span.

On July 24, mine experienced an adverse geological event, including fall of roof in alive workings, that impaired ventilation or impeded passage within the 6th Sub Panel East off 1st South Panel. No injuries were involved and MAHA was contacted within 15 minutes.

A roof fall has occurred at Shell Branch Bleeder in #4 entry at crosscut #14, the size of the fall was 20' x 20' x 8' which involved a 14-9 remote control continuous miner. The area will be cleaned and additional roof support will be installed.

The hoisting engineer detected a distortion of unknown origin in the slope hoist rope approximately 30 feet outby the connection to the slope car. The hoist was shut down, MSHA notified and the mine evacuated. The rope was repaired and returned to service.

A transformer fuse blew on a 3 phase conductor disabling the #2 hoist. The mine was evacuated using the #1 production hoist. The evacuation started at 12:15 a.m. and was completed at 12:25 a.m. The #2 hoist was restored to service at 1:35 a.m. The hoist was tested and miners returned UG at 1:55 a.m.

A roof fall occurred in the 10th Right Section, #2 Entry, #7 crosscut, in an intersection. The fall measured approximately 17 feet wide by 16 feet long and approximately 8 feet high. The roof fall was reported to the MSHA Hotline.

2 ee working with a DC powered welder and oxygen/acetylene touches to repair a cutting bit lug on a continuous miner. Rubber hoses of the cutting torches accidental came in contact with the hot case of the bonder, melting the hoses and igniting the gases, causing a fire. Fire lasted 10 to 12 min was extinguished with water. No inj were reported due to this fire.

On July 22, 2013 a maintenance mechanic was using a cutting torch to remove wire mesh that had been roof bolted in place covering a core hole. The use of the torch ignited a small quantity of methane gas that had collected close to the roof / back.

A roof fall was found in our M4 Mains, x-cut 64 in a neutral entry adjacent to the belt line. The fall was 25' x 20' x 12'. The fall did not affect ventilation, impede passage of miners or cause harm to any miner.

An unintentional roof fall occurred in the Main North Return xcut 37 footage marker 5592 entry #2. The dimensions of the roof fall is approximately 17.5'W x 12'T x 40'L. The fall will not be cleaned up and a plan was submitted and approved to re-route the lifeline to #1 entry. Roof fall #30 - C.

Accident type, without injuries

Employee made contact with supervisor to inform of being in an inoperative elevator located in the Empire Crusher 1 Building. Supervisor responded to the site to try operating control buttons. The elevator remained inoperative, so a service tech was called. About 4:00 am MSHA Hotline was notified of incident. Service tech reset the elevator and employee was ""freed"" at 4:18.

A roof fall above the anchorage was discovered in the Main East Return at wall number 12-27. The measurements of the fall are 30 feet long X 16 feet wide X 12 feet high.

A passing Union Pacific loaded coal train emitted sparks and ignited a grass fire. There were no injuries or property damage as a result of the fire.

Unplanned roof fall occurred on 7/18/2013 in the SW Mains 3. There were no injuries or equipment damage due to the fall and there were no escape ways blocked or ventilation disruptions. The area will be dangered off and it will be cleaned up at a later date.

Electrical switch on the elevator door malfunctioned. This made the elevator inoperable. Elevator became operational at approximately 6:30 AM

9:05 p.m. employee reported smoke coming from the rotochopper. No personal injuries. It was discovered that the discharge conveyor broke causing the rotochopper to plug up. Smoldering material was found at the discharge of the rotochopper.

While doing preshift examination discovered roof fall out by in 4-5 entry at spad # 7961 mearsuring 30 ft.x 15 ft.x 10-0 ft. high

An unintentional roof fall has occurred at the Old #1 Horse Creek Mains 154 spad in the #5 Entry. The roof fall was approximately 50ft x 20ft x 5ft. No injuries were reported and no equipment was involved.

A roof fall was discovered by the mine examiner while traveling the return air course in #2 entry inby spad 450. The estimated size of the roof fall is 30'long, 20'wide , and 8'high. No injuries were reported as result of the roof fall.

A roof fall was reported to MSHA on 07/15/2013. The incident occurred at 27 block, crosscut 2-1 of the 7 Right section, outby the tail piece. The affected area measured 20' long and approximately 4-5' high. Upon further investigation, the operator does not feel that this is a roof fall nor does it meet the requirements of an immediately reportable event.

A roof fall occurred at 27 crosscut on the 4 North track entry. The fall is 65 feet long, 17 feet wide and 12 feet high. The area is re-supported.

At 8:05am the #1 elevator car had left the bottom landing and traveled about a foot. Nobody was on the car. There was a problem with the DB contactor. The defective component was replaced, the car was tested, and returned to service at 11:50am.

A roof fall occurred at 38 crosscut on Cool Hill #2 belt at 38 crosscut. The fall was 15 long, 12 feet wide and 8 feet high. Weathering was the cause.

Shift supervisor was called to the 5265-516 by the jammer operator to look at the condition of the heading. The last wire and bolts were hanging in the heading that was being jammed up with cement. The shift supervisor thought it was a ground fall because the bolts were hanging out.

Between 8:00 p.m. and 12:a.m. Appears to have been an electrical fire that started in the electrical junction box. Damage to locker and meeting rooms. Smoke damage to entire trailer and contents.

Wana Service Elevator went down with electrical problems. No person on board. After an investigation contactors and hoist motor were replaced. Hoist back in operation 4:15 P.M. on 7/14/2013.

The elevator would not function as the ceiling door switch was not activating to allow the elevator to function, as the door was not completely closed. The door closed. The elevator functioned, no damage found.

An unintentional roof fall occurred on the 002 MMU. The fall occurred in the #5 face, approx. 100 ft. inby spad #14204. It measured approx. 40' x 20' x 8' to 9' thick.

An ignition incident occurred during the relighting of #4 kiln after an electrical storm. The Raymond coal mill was cleaned out of coal dust but when the mill was started, coal dust remaining in the mill ignited inside the kiln. No damage or injury resulted from this incident.

A fall occurred at the 6 and a half crosscut of the #4 entry (120' outby the loading point). The area was bolted with 8' install two and 16' cc cable bolts. No ventilation, equipment, or people were affected.

At approximately 2:55 a.m. an incident occurred involving a dozer operator who inadvertently entered the number 4 feeder located at the raw coal pile. The employee safely exited the dozer uninjured. Job skills and miner's proficiency may be a factor.

The #9 Portal elevator was out of service from 5:10 pm until 6:00 pm when a fuse on the bottom door switch blew. The fuse was replaced.

We had a roof fall 7' to 8' high, 18' wide and 22' long in a 4 way intersection on c-panel in # 1 entry @ spad # 4+20. This is the belt entry for the c1-panel.

An unplanned roof fall occurred on 6A belt in the #5 entry. It fell in an intersection measuring 20' long X 20' Wide X 3 1/2 X 4 feet high.

The fall was discovered at the D-Sub Section (#11 Crosscut, #6 Intersection). The area was bolted with 8' combo bolts and straps with 12' cable bolts. No equipment, ventilation, or people were affected.

An unintentional roof fall occurred on 3D Belt line XC-110 Entry 5. The mine was idle for the Holiday weekend and was conducting a fire check when the fall was discovered.

The # 1 elevator was taken out of service at 10:15am for scheduled maintenance. The govenor rope was changed and the car was returned to service at 1:15pm.

Roll back fault occurred in the service hoist system. Hoist was reset and all checks were done. Hoist was placed back into service.

An unplanned roof fall occurred at XC78 in entry #9 on the Main North. The fall measured 20""Lx18'Wx5""H, limited to the intersection. There were no injuries and no equipment involved. The fall was in the intake escape way and required the lifeline to be re-routed. The lifeline has been re-routed and supplemental roof support has been installed.

At 5:30 AM on 7/4/13, a brush fire was reported within the quarry boundary. Incident commander was notified by radio & ERT members were dispatched with a brush & water truck. USFS was notified & they responded with a fire fighting crew. The size of the burnt area was estimated at 0.25 acre.

The Crabapple elevator was not operational for an extended period of time. The proximity switch on the top of the elevator guides was replaced. No injury or other equipment damage as a result of this incident.

During a routine test trip the brakes set and would not release because the system could not build up the required pressure. Through investigation it was determined that the relief (pilot) valve had failed due to contaminates within the oil. Valve replaced, and oil samples will be taken regularly.

An unplanned roof fall occurred in entry #2 between XC 35 and XC 36 of the West B off West A, off 2nd West submain off Main North. The fall was in the return entries, there was no active mining inby the fall. There were no injuries and no equipment involved. The fall measured 90' L X 18'W X 10'H. Ventilation for recovery work was disrupted. Ventilation has now been re-established.

An unplanned roof fall occurred in an intersection in the #1 entry of Tilley Mains in front of the #2 seals at spad location 0275. The fall measured 65' long X 18' wide X 7' high.

Main hoist found inoperable at 7:00am. No employees underground. Reported 9:04am to MSHA. Employees entered mine approximately 10:00am maintaining 2 escapeways. Timer relay on hoist PLC was replaced and put back in operation at 8:50pm.

A ground fall breaking beyond the anchorage involving the foot wall rib and a portion of the back occurred in the 6-5530 stope. No one was in the area when the failure occurred.

The Cat 773E Rock Truck started having a problem with the right front strut. The mechanic was called and was conducting Trouble shooting procedures on the truck when the strut failed causing crushing injuries to the mechanic.

Operator working at site pushing material and apparently went over earthen wall into water.

Drowning

EE was cutting grass with 7300 compact track loader w/attached brush hog. For some unknown reason the machine lost traction and slid down small embankment into water. Cause of incident unknown at this time. Compliance with rules/reg not a factor, mining equip/system unknown to be a factor, but was used, PPE not a factor, job skills/miner proficiency/training/attitude not a factor.

Caught in, under or between a moving and a stationary object

Employee was fatally injured when he was struck by a battery powered coal hauler in Unit 2.It is believed that the employee walked through the last open crosscut to #4 entry and sat down near the inby corner of #4 entry to eat his lunch. A coal hauler rounded the corner and pinned the employee to the rib.

Over-exertion in lifting objects

Injury was not reported until 9/6/13. EE alleges that he felt a pull in his stomach while adjusting the under belt on the twin screen. He has been diagnosed with a hernia and released back to full duty but will require surgery at a later date. The accident date is a just a guess by the EE.

Struck by falling object

EE was contacted via radio by the leadman that the 834B had broken the drive shaft. The EE returned to the machine and placed a bar on the steering cylinders and ratchet strap around the bar and the drive shaft. While attempting to position the drive shaft for re-assembly the strap and bar fell causing the drive shaft to fall onto the right ring finger severing the end.

EE was working on a part clamped in a vise. When he loosened the vise to reposition the part it began to fall and EE tried to catch the part and it mashed the tip of his middle finger on his right hand causing him to loose the tip of his finger at the first joint.

Struck by flying object

While operating a ram car during loading from the face, an undetermined object was propelled into the employee's left eye, resulting in the loss of sight in the eye. The employee was wearing eye protection, but had the protection lowered.

Caught in, under or between a moving and a stationary object

An employee was injured while attempting to catch a light pole that was being re-established. The employee's right hand (thumb tip) was pinched between the light pole and the handrail causing the pad of the right thumb to be disconnected from the right thumb.

Turned machine off and tightened belt. Turned machine on to see if it was working and got the tips of his right index & middle finger caught.

Fall to the walkway or working surface

He was walking on top of an old overcast. Loose panels in the overcast roof slid out of place and he fell through the overcast landing on his left arm and wrist. The result of the injury was a fractured radius bone and ulna bone. these fractures required surgery.

The man was replacing the splices on 1st East Mains belt line with a hand operated splicing machine. The man has been off work since 8/12/13.

EE was assisting contractor with removing brake assembly on CAT 990 Loader. While in compartment, the prop rod for the compartment door became unlocked & a gust of wind blew the door down, striking the EE on top of his hardhat. The EE experienced neck pain from this incident. IME report on 10/16/13 changed diagnosis to cervical radiculopathy requiring physical therapy treatments.

Threw a rock off the arm of the boom, twisted his back. (Strained lower back).

Employee was changing out Diamond drill head on the drill. As he was taking off the old head it slid/moved & caught is R. hand in-between the DD head & water tank, causing contusion. Xrays were negative, but restrictions were issued.

he was bringing a roller to change it and his knee gave out.

Over-exertion in lifting objects

EE shoveled during cleanup and stacked 50 pounds bags onto pallets and strained knee while doing so.

Employee was preparing to weld the left wing section of a haul truck bed to the main section of bed. He was readjusting chain when crib kicked out causing the left wing section to turn over pinning him to the ground.

Fall from piled material

Employee stated that he was holding timbers that were being cut by two other employees by sitting on top of the timbers. As he dismounted the stack of timbers, he got his foot caught in the bindings that hold the timbers together and fell to the ground. Medical received on 08/15/2013 diagnosed the employee with multiple sprains/strains.

While loosening conventional binder in preparation to unload loader bucket on trailer, swede kicked back and struck lower chin. Lacerating lower chin, fractured mandible and dislocation/loss of lower front teeth.

Employee stated he was re-hanging curtain in the #6 heading when he lost his footing and fell onto the scoop bucket hitting his left wrist against the machine.

Fall from machine

Employee missed last step while dismounting a company tractor. (MM 20244). Employee fell on left arm and sustained multiple fractures.

Injured employee was walking next to the longwall shearer as it was cutting coal on the longwall face when the shearer kicked out a rock that struck the employee's left thigh area resulting in a bruise.

Changing shield relay bar on long wall shield, shearer went by and pan line pushed out and the shield was not all the way off the top, the hog chain pulling it broke and hit him in the bottom of the foot. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency and communications was a factor.

While Operating shear, rock fell between the shields, striking him in the back of the head. He began missing work on 8/1/2013

Employee was operating the continuous miner (CM) and he noticed a large piece of draw rock resting on the top of the CM. He was attempting to remove the draw rock and the rock broke loose, sliding toward the employee and striking and fracturing his left foot. He started missing time on 7/31/2013.

Employee stated that his shoulder had been bothering him for some time. After review from a physician it was determined that employee would need surgery to correct his shoulder. This will be degree six.

Hanging phone cable, when he stepped in a hole causing him to fall. He caught himself with his right arm, and felt pain in his shoulder. He continued to work until 8/27/13.

Miner was removing damaged hood from haul truck. After removing pins from hinges, miner was removing tension rod with hammer when it came loose and sprang up and made contact with miner' s hand.

While operating a scoop, employee mashed three fingers on his left hand.

Over-exertion in pulling or pushing objects

While bending a six foot bolt, the employee felt a sharp pain in his right shoulder. This resulted in a shoulder labral tear which resulted into a lost time accident.

Slipped and twisted right knee while pulling rock dust hose onto belt line.

Employee didn't realize there was a stone on the conveyor belt, stone came off belt and crushed his right ring finger between another stone.

Employee was going to replace a cut off valve for water on shield 115 when he turned a high pressure valve on and was struck in the face by the hose requiring sutures.

Over-exertion in lifting objects

The employee stated he was lifting muddy line curtain and felt pain in his left shoulder. The employee sought medical attention and received physical therapy, but did not miss any work. After further consideration, this is being filed as a medical reportable.

Fall onto or against objects

employee (on ground) was looking for material located on opposite side of row of scaffolding. Rather than walk around, he attempted to cross under scaffolding, tripped on a cross bar and fell resulting in a contusion on his back.

Employee was moving the continuous miner to set up for daylight. While the employee was moving the miner, he twisted his ankle and sprained it.

Struck against a moving object

Employee was operating a rock truck when he encountered rough terrain, causing the truck seat to bottom out, resulting in neck and back pain. The employee began missing work on August 5, 2013, as a result of this injury.

EE states he was cleaning the back side of belt, breaking a large rock with hammer and a small rock hit him in right eye. while talking to EE stated that he was not wearing safety glasses.

Contact with hot objects or substances

Employee was starting to remove cutter shaft on longwall shear and when he took the bolts and snap ring off oil came out and burned both his lower arms and both hands.

Fall to the walkway or working surface

Employee pulled a piece of draw rock. When he started to clear the rock from the roadway he twisted, then stumbled and fell. He experienced a sharp pain in his back and down his left leg and knee.

Fall from machine

Employee states he was dismounting his dozer a D11 R and slipped on the step beside the engine compartment and fell between the dozer and tracks. On 7/28/2013 employee was treated at Boone Memorial Hospital by Dr. and diagnosed with a lower lumbar and left shoulder strain and taken off work. This is now a lost time injury.

An accident occurred during routine maintenance on C10X10-30 Dredge. The pump shaft assembly was not properly rigged to prevent hazardous motion. The assembly slipped when lifted, striking and knocking the employee to the deck. The assembly landed on the employees leg resulting in two broken bones.

Employee worked overtime & ate lunch provided by Noranda. Began feeling ill and thought it was a stomach virus. Upon returning home from work, the symptoms worsened and he was admitted to a hospital. He was diagnosed with colitis. The physician stated that whatever he had eaten 8 hours prior to his symptoms starting is what made him ill.

No witnesses were present at the time of fall.

While checking out the tail roll on the #12 belt due to a conveyor/chute plug up a rock fell off of the catwalk above him striking him on the side of the head. This incident was a result of a plug up which caused limestone material to back up in the chute and therefore spill onto the catwalk above.

NOTE: Suspect fraud, see attached statements from men that worked with him. When mine started laying off, he alleged injury on 7/30/13, but did not miss until 9/19/13, almost 2 months after the alleged injury! Mine laid off contractors 12/4/13. Job ended.

The employee was jerked forward into the seatbelt. he went to the doctor, was given medication, and returned on his next scheduled shift. **On October 14, 2013 the employee was reassessed and assigned modified duty due to issues with his leg. Working on pad crew, ee stepped on HDPE pipe main line with left leg. Pipe rolled causing employee to fall on to left knee. He was seen at the ER, referred to Dr. where he was diagnosed with a soft tissue tear and scheduled for surgery.
