Shift supervisor was doing his daily round and went into the 5432-110 and found a ground fall had taken place. The area was approximately 30' long and 12' high and 15' wide. No equipment or personnel were working in the area.
A roof fall was found on the South Mains 003 unit spad #15010 extending out by 210'. A MRS cable was caught by a piece of rock.
A weekly examiner identified a roof fall on 8 South, #3 entry at #13 crosscut. This roof fall measured approximately 10' X 10' with a distance of 16,189' from the nearest working face of the mines. This area is not normally traveled by our employees and does not interfere with air or travel ways.
Hoist became inoperable at 8:15 p.m.. Upon investigation it was found that switch synchronization would not occur properly. Programming was corrected making the hoist fully operational at 11:45 p.m.
A roof fall occurred in #7 Entry of the 2 West Mains panel above anchorage. The fall was located at spad 1109.

A roof fall occurred in Entry 4, crosscut 52 of the 2nd West Sub-Main. The fall measured approximately 24' wide by 35' long by 8' high.
A non-injury roof fall occurred at survey station #2751, No:3 Entry.
Kuhntown Elevator stopped 200' below ground level with 2 persons on board. The 2 men were removed safely 37 minutes later with no injuries to report. Elevator was tested by Schindler and put back into service with no reported causes.
A HEAVY STORM CREATED 65-80 MPH WINDS. THE WINDS BROKE A WOODEN POWER POLE LOCATED ON US 40 HWY 2 1/4 MILES NW OF THE MINE. POWER WENT OUT AT MINE WHEN POLE BROKE. POWER OUTAGE OCCURRED AT 5:50PM, MINERS NOTIFIED TO EVACUATE AT 5:52PM, MSHA 800# WAS CALLED AT 5:58PM. MEN ARRIVED ABOVE GROUND AT 6:20PM USING ESCAPE SHAFT. POWER RESTORED AT 6:30PM.
Waste dump was found to be on fire. No damage or impairment due to this being a waste dump (cardboard and pallets).
A roof fall was discovered at 1:30pm in the #4 mains one xcut in by SS 651. The fall measures approx. 20' x 20' x 5' high.
A roof fall was discovered in the Northwest Mains near Crosscut # 84. The roof fall measured 4 feet by 10 feet by 60 feet. There were no injuries and the ventilation was not interrupted.
The #8 Elevator stopped within 12 inches of the top landing with one passenger on the car. The car showed overspeed fault. The rope gripper set and locked the car out on fault. The passenger was stuck for 5 minutes. The car was reset and tested. The agencies were notified. The elevator returned to service at 3:45am
The inter coder was not functioning and needed repaired. Hoist became in-operational at 6-25-13 5:10pm and was fully functional at 6-25-13 5:39pm.
A fall of ground occurred at the face of the 4510-498 heading. A mucker was in the process of squaring up the face when material fell from the back and onto the bucket and arms of the loader.
1200 Hitachi excavator had a hose burst, spraying oil onto the engine starting an oil fire. The operator then used the fire ext to remove himself from cab.
An unplanned roof fall occurred on the #2 section in the #3 entry 1 break inby spad 4359 in the intersection and extends 20' over to entry 4 measuring about 35' long 20' wide and 8'-9' high
The surface supply track hoist rope cable became kinked when the system was placed in automatic to lower supplies into the mine and the track switch on the surface was left thrown for the surface supply yard and not the slope track. This allowed the hoist rope to overlap and kink in the slack that was created. The overlap switch was found to be bad.
An unplanned roof fall occurred on the Stallion mains inby spad 3973 located in the face on #2 section. The falls specific location is in the last open crosscut from #3 entry to #7 entry. It starts from 1 break inby #3 entry and runs through 6 right crosscut.
Power was lost to spider and needed to be restored. Four miners were trapped. There were 111 US Steel employees working at time of entrapment and 24 contractors. MSHA was notified at 4:56 p.m.
The portal passenger elevator went down at approximately 8:16 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The elevator was returned to service at approximately 10:03 AM. A defective door switch, which allows the elevator door to shut, was found to be the problem.
In the #3 shaft near the 900 ft range, a torch was being used to remove old guides in preparation to install new. During this process a piece hot slag fell from the work area and lit a known methane bleeder. A sample of the gas was taken for analysis, and a permanent solution is currently be devised to prevent reoccurrance.
While mining towards the headgate near #68 shield, the shearer came over a roll in the pavement; This caused the head drum to cut into the pyritic mine top which resulted in an alledged ignition.
An unintentional roof fall has occurred at the old 7A Section #3 #6 Belt Entry outby spad 13633. The roof fall was approximately 44ft x 19ft x 5ft to 6ft. There were no injuries reported and no equipment involved.
On 6/21/2013, the Brakeman Car (Bonny Car), in the slope needed charged. The call was made out of miscommunication and there was no physical damage to the equipment. This form is being filled out to give notification of the actual event information.
Dozer cut a pressurized gas line which ignited a fire. Whitesville Fire Department extinguished the flames.
An unplanned fall of the mine roof occurred on the 7 Right section of 1 North Mains at 5 block, crosscut 1-2. The fall measured approximately 50' in length by 16' wide and was in excess of 6' in height (above the anchorage of the installed roof bolts). Ventilation was not impaired and the escapeways remained open.
EE saw a small glowing ember in the lower screen of the sifters while preparing to install a new torque arm. EE discharged two fire extinguishers into glowing ember, but was unable to extinguish the fire. He determined it was unsafe to remain in the mine, and evacuated the mine. No injuries or illnesses occurred.
A contractor from ACME Constructors was welding and caught a belt on fire. The incident was immediately reported to MSHA.
On 6/20/13, a surface security guard heard a noise outside.The guard observed a highwall failure at the No. 2 KY portal. The guard notified management. Mgnt. arrived at the mine site, observed the highwall failure and notified MSHA. Upon investigation by MSHA it was determined this event was a highwall failure and also a roof fall at the entrance of the portal.
A roof fall above the anchorage was discovered along the #14 Mother Belt at #24 crosscut. No injuries, ventilation not affected. Travel in the area was impeded. The dimensions of the fall were 25 feet in length, 21 feet in width, and 12 feet high.
A roof fall was found in the 2nd North panel blocking the No. 1 and 2 entries, the No. 2 unit right return. There was also a fall in the No. 3 neutral entry. The location was at spad No. 40+83.
An approx. 50' long section of shotcrete applied to the left rib in the 4675 NE Lateral sat down as a result of the shotcrete being undercut along the sill and no longer binding to material on the rib. The shotcrete remained vertical and against the rib but pulled away from just below the springline. Did not pull above anchorage. Neither ventilation nor passage was impeded.
Truck was following behind another rock truck when the truck in front of him stopped to allow a loader to turn around in front of them. The truck in the back could not stop and struck the front truck in the rear end. Neither driver was injured. EE was asked to go to hospital but refused.

Roof fall above the anchorage was discovered at 5:08 pm on June 19, 2013 in the Maple Avenue area of Old Main North. The fall is 39 feet long by 16 feet wide by 9 feet high. The area has been re-supported as per the approved roof control plan.
An unplanned roof fall occurred at 4:10 am on 6/19/13 on the MMU-001 section in the last open break in the connecting crosscuts between #5 and #7 entries. MSHA and State were notified immediately. Roof fall did not affect ventilation or passage of persons, support consisted of 6' glue bolts. Fall was rehabilitated.
An unintentional roof fall was discovered by the pre-shift examiner in the number two entry at 16 crosscut on longwall 61. The fall included the entire intersection and fell above the anchorage point of installed roof bolts.
The #8 Elevator was removed from service at 2:45 PM. The bottom door lock failed. The bottom door lock was replace and then the car was tested. It returned to service at 4:45 PM.
The Volvo 220F loader had a fire in the engine compartment that destroyed the loader.
An electrical pole with a cracked insulator located at the top of the pole shorted out when a bird landed on it. The birds feathers caught fire and fell to the ground catching the grass on fire.
Upon entering the section, maintenance crew observed a roof fall occurred. Section Foreman called Mine Foreman who notified MSHA. Fall occurred 1 cross cut inby the loading point of entry #5 in Gold Mains Section. 40' outby survey station 753. Measured 30'L x 24'W x 14' H.
An unplanned roof fall occurred at crosscut 165, on the 5th 54 beltline in the #11 seam. The roof fall is located in an intersection and will be cleaned up.
A roof fall occurred at the intersection of E. Corridor #1 entry and C-Mains #2 entry at 60 x-cut of E. Corridor. The fall was approx. 10'-12' high across the intersection. There were no injuries. The area was dangered off and posts were set on all 4 sides of the intersection. Ventilation was not effected. The alt escapeway for the C district was re-routed around the fall area.
The new pinion brake that was installed on the #2 hoist would not release. They had to be released mechanically. Upon initial investigation by the design engineers a power surge was the cause.
A rockburst occurred in F entry of 4600 ramp which bagged the wire mesh & sheared a split set bolt. This allowed a small amount of material to be released(apprx. 2 1/2 yds). There was a 6x8 section of the crown about 50' from the face that was heavily damaged and this is where the material was released. The wire mesh contained about 90% of the material
Roof fall - 16'x35'x6' to right of slate 5948. Intake 5 block, xcut 5 to 6 along 2 belt.
An unintentional roof fall occurred in a wet weather area. The fall measured approx. 30' x 19.5' x 11' . The area has been cribbed off.
During the weekly mine examination a roof fall was observed in an intersection located in the old 1-south sub-mains in No. 3 entry, one crosscut inby spad No. 3865. This is a non-active area of the mine. The fall measured approximately 18 feet wide by 40 long and 6 to 8 feet high. No travel blocked, no ventilation blocked and the fall will not be cleaned. Only breakers posts set.
An unplanned roof fall of the mine roof occurred on the 7 Right section of 1 North Mains at 1 block, crosscut 1-2. The fall measured approximately 50' in length by 16' in width and was in excess of 6' high. Ventilation was not impaired and both escapeways off the section remained open.
An unplanned roof occurred in the intake air-course/intake escape way one crosscut from spad #44. The fall is the entire width of the entry. Height - unknown. There were no injuries or entrapment. Blocked primary escape way but escape way has been rerouted.
Removed #1 Elevator from service at 2:00 PM. The counter weight buffer was found to be leaking. Schindler was contacted and removed/repaired the buffer. The car was tested and returned to service at 8:00 PM on 6/15/13.
Propane gas buildup and ignition causing substantial damage to bucket elevator.
In the A2 Area (Pit #45) there was a dirt spoil slide that disrupted the mining process for more than 1 hour.
A fall of roof occurred in the 2nd South east Submain, Entry # 5, # 105 crosscut. The fall measured approximately 50 feet in length x 18 feet wide x 10 - 12 feet thick. The area was supported with 6 foot fully grouted roof bolts.
A roof fall occurred in the #2 entry along the sturgis mains at spad #93+68. The fall is approx 3000ft to the nearest section. Fall measured 18.5' by 35' by 12' plus high.
Removed #1 elevator from service at 10:45am to conduct scheduled maintenance on the governor. Completed the preventive maintenance. Tested the governor and returned to service at 1:00pm
AT APPROX.7:35 AM THE MINE EXAMINER FOUND A FALL AT X-C 32 ENTRY # 7. THE LOCAL FEDERAL INSPECTOR ISSUED A K ORDER AN EVERY THING WAS COMPLETED AS ORDERED BY THE INSPECTOR TO LIFT THE ORDER.
An unplanned roof fall occurred, there were no injuries or entrapment. The location of the fall was in the return airway at the #11 & #12 northeast main seals. The #11 seal was completely blocked. The measurements of the fall were approx. 18' wide, 14' high at a length of 60'.
A fall was discovered at #3 Mains Section (#15 Block, #4-#5 Crosscut). The fall was 13,760' outby the closest loading point. The area was bolted with 6' SuperTwist bolts. Ventilation, people, and equipment was not affected by the fall.
The #2 hoist had to be shut down because a loose hydraulic fitting sprayed fluid on the motor of the hoist. We were installing a new pinion brake and a mechanic forgot to tighten all the fittings. When we attempted to check the new system, the now charged hydraulic line sprayed fluid out of the coupling.
A fall was discovered at #3 Mains Section (#15 Block, #3 Intersection). The fall was 13,800' outby the closest loading point. The area was bolted with 12' cable bolts and 6' SuperTwist bolts. Ventilation, people, and equipment were not affected by the fall.
Employee was washing out a misfire when the cap detonated causing rock to shatter windshield and door glass. No injury
Took elevator out of service. Stopping by itself on up from bottom. Four door tracks had pushed up due to rust. Corrected problem and put back into service at 5:05PM.
At approximately 11:10 pm the service hoist went down due to communication fault. It was found that one of the communication cards in the PLC was not making good connection.
Conditions: Sunny, stable ground. The fully loaded tractor trailer with broken concrete was discharging load when a portion of the load was lodged at the driver side front of the trailer. The offset load caused the trailer to overturn towards the drivers side. The overturn resulted in damage to the tractor trucks drivers side of cab and twisted frame. Operator was not injured.
On 6-07-13 the North side chute had an issue causing operations to be put on stand by at 12:35 am. Trouble shooting found the N. chute trunion pin flipped. Operations were halted at 1:10 am and as a precaution MSHA was notified that hoisting would be stopped and the miners would be extracted. All were out in twenty minutes. No miners were trapped or injured.
A Cat 992G Loader was loading a truck and a loader tooth fell into the truck. As the truck dumped the rock into the primary crusher, the loader tooth got caught between the mantle and the crusher liner stopping the drive. The primary crusher motor did not stop and high friction on the pulley caused the belts to start to smoke.
At approximately 6:15pm the Shindler became inoperable. The door switch was adjusted and the elevator was back in operation.
The door switch for the 8 Haulage elevator shaft, our secondary escapeway, malfunctioned.
A roof fall was discovered on at this time. The fall was located on C-West, #2 entry, at crosscut #2. The fall measured 25' by 30' and was 6' in thickness. Ventilation was not affected, nor were any employees. The area was originally supported with 5' H/Tech bolts and 12' cable bolts.
A non injury roof fall occurred on the 3rd Northeast Mains in the #6 entry intake split approximately 150 ft. outby survey station # 184. The fall was approximately 50ft. long x 20ft. wide x 7ft. thick.
Unintentionally mined into an uncharted well on the E-24 Longwall face. No Damage.
The drive card for the motor for the 8 Haulage elevator, our secondary escape way, shorted out and was replaced.
The clutch on #4 Ball Mill failed, causing the internal rubber bladder to burn. The contractor who discovered the fire did not report it to company personnel until after the fire was extinguished, resulting in estimated timeframes. The fire may not have lasted 30 minutes.
A ROOF FALL WAS DISCOVERED WILL MAKING AN WEEKLY EXAM OF THE RETURN OF THE 2ND SW MAINS. THE FALL IS IN THE #8 ENTRY. IT IS APX. 40'LONG X 20'WIDE X 8'HIGH. THE FALL DID NOT AFFECT MINERS, VENTILATION NOR EQUIPMENT. THE AREA WILL NOT BE CLEANED UP AT THIS TIME. THE AREA HAS BEEN TIMBERED AND DANGERED OFF TO PREVENT ENTRY.
Roof fall was found in the return entry of C Main in #2 Entry and 2 to 1 Entry - Fall was appox. 35 x 55 x 4"" thick.
We had a roof fall in the Headgate #2 bleeder entry #2 between x-cut #9 and #10.
The Hoist operator experienced a brake wear fault on Hoist. The brake wear indicator failed. The linear potentiometer is located and secured on the rear arm that is attached to the counter weight. It cycles on a hydraulic style cylinder which tells the hoist of the brakes need replaced.
Miner was attempting to bring disabled truck down from the upper most harvest area. As he was dumping load, breaks let loose & truck went down uncontrollably. As it came to the next lower level, it hit a berm causing the miner to be ejected through the right hand window & was presumed to have been struck by a part of truck.
Employee walked up to the 7-2C header to inspect wipers. The victim was on an elevated catwalk at discharge of the 7-2B conveyor belt to examine the belt wiper. He fell off the end of catwalk on 7-2B belt. Employee was caught between the skirt and the top belt resulting in a fatal injury.
Employee was in the procees of building a berm when his loader went into the stope.
While stabilizing a portable crusher the jack slipped out from underneath it, and it fell on the employee's right arm.
Employee received minor break in the skin to his left small finger, while not wearing gloves. 6/23/13 the employee was taken to the emergency room due to an infection in his left small finger. The infection worsened and eventually the doctor decided to perform a partial amputation to the second joint on his small finger.
Employee stated that he aligned his screen on the roof, his steel was started on a strand of the screen. When he rotated the drill steel to drill his hole it wrapped the screen around the tip of his finger pinching the tip of his left index finger off.
While installing 20' long panels on an overcast 4-breaks outby #9 drive, all five crew members were picking up panels and sliding them over the overcast walls. Two contractors were pushing a panel into place and employee could not get out of the way. The panel caught his left hand and middle finger. This resulted in ee left middle finger being amputated just below the knuckle.
Miners were replacing the cable that controls the extension and retraction of the telescoping end of a stacker conveyor. The telescoping end of the stacker moved causing tension on the cable. A miner's foot was caught in the cable resulting in a broken leg. The miner was airlifted to a local hospital. The miner's left foot was amputated just above the ankle.
Employee was installing support arm for canopy and while using come-a-longs the chain came out of the clevis allowing the canopy to fall hitting employee's finger causing the tip of finger to be cut off.
Employee was assisting the emptying of dust collector hopper & unchoking tipping valve when he reached inside of the tipping valve to remove a piece of material. The screw conveyor below the tipping valve removed middle fingertip. Employee returned to work after 1 day away from work.
The worker was doing contracted conveyor belt splicing & was kneeling by the belt when he turned to get a hand tool, twisted his knee, heard a pop. He worked for the rest of the day without reporting the incident to his supervisor. He also worked every work day through 7/15/13 & reported the incident on 7/16/13. He's been out of work since 7/17/13 & has filed a workers comp claim.
Employee was ""loading dynamite"" underground when he heard a ""rock cracking"" noise, so he looked up and a rock that fell from above hit him in the face. His right front tooth was dislodged and he had minor lacerations above his upper lip.
Injured was taking hose out of dolly the hose jumped out pinning hand between corner of dolly and high pressure hose. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency and knowledge a factor.
Employee was getting out of skid steer, slipped on step and fell fracturing his left forearm. Ground was muddy.
While working as a rib bolter in the BMX Mains Right Section a piece of Rib measuring 56"" X 30"" X 14"" struck the employee on his left side and ankle. This caused a fracture to the employee's left ankle.
EE (owner) wall of rock fell on him. Broke both legs, feet, broke vertebra, broke right wrist
Employee was raking up rocks around a beltline as the plant was preparing to shut down for the end of the shift. Employee went to pick up a rock on the ground and another rock bounced off the conveyor belt and hit the top of his index finger on his left hand which caused the tip of the finger to split open which required stitches to close.
The Prep-laborer reached with his right hand to remove a steel choker while a splice house was lowered onto a lowboy trailer. The cross brace of the splice house and surface of the trailer pinched his right mid-finger. The finger sustained an avulsion requiring sutures to close the wound.
The individual was exiting the BT-24 mantrip when his foot became caught causing him to fall to the ground striking his left side. He is complaining of pain of the left hip, shoulder, pelvis, and back area.
An employee was getting 203 loading shovel's cable off of a cable tree. The rope between the tree and the shovel had tension on it. He stepped on the rope and then pulled on it to try and remove the clevis. This is when he heard and felt a pop in his right arm. Employee was transported to ER where he was diagnosed with a right bicep tendon tear.
Injured employee was using an axe to cut nylon bands on a bundle of cribs on the 17 Left Longwall when he struck himself on the left shin causing a fracture. Employee finished the shift and began missing scheduled days the following shift.
The employee was using a hand held grinder to remove a broken nut. He was wearing gloves and his left gloved index finger was caught in the grinder disk. He pulled his hand out of the way and received a laceration from the guard which required stitches.
Employee stated that he was assisting with moving the CM cable out of the roadway. As he lifted the cable and twisted to throw it out of the way he felt pain in his right hip. Employee continued to work until 08/08/2013 when he was diagnosed with a fracture of the right femoral head.
Employee was retrieving a mine de-watering pump, when the rib sloughed/rolled out and struck the employees hand against the pump, causing injury to his thumb.
Employee was walking up a hill with a tool box. The tool box broke, causing the employee to fall to the ground.
Employee was trouble shooting the tram switch on a rubber-tired mantrip when it moved unexpectingly causing injury to his right arm.
EE was standing on face side of LW belt at -1 block directing his EEs while pulling end of belt they'd just installed a splice on. They'd attached a side grip on face side & pulling w/DC winch. EE knelt from a position 8' inby the grip instructing winch Op. to keep pulling when grip slipped off belt striking the bottom of a belt stand then EE's lt forearm & the top of his hard hat.
Employee states that while bolting top, he went to put the drill steel in the pot when a piece of draw rock fell from the top and struck him in the left forearm.
Employee received flash burns to his left eye while assisting in maintenance. A prescription eye drop was prescribed and an additional eye covering bandage. EE missed 1 day of work.
Employee was operating a D9L bulldozer and he states that the seat belt was aggravating his right groin. He had right inguinal hernia repair surgery in March 2013 and went back to work in early June. He had noticed increasing pain in his right inguinal area with swelling and ? numbness.
Truck was loaded at pit. Driver was leaving pit and though truck was in lower gear. Tried to down shift and could not. Applied brakes but was not able to get back in gear. Picked up too much speed so she steered into bank. Truck and trailer flipped into irrigation ditch.
Employee was setting a jack pipe and when he let go to the mud seal for the top of it, the mud seal came down and smashed his small finger right hand between the seal and screw jack handle.
Employee was performing task inside screen. When he entered the screen he was wearing his safety glasses. Due to heat and humidity, he could not keep his glasses from fogging over and removed them to complete his task. While he had his glasses off, sand fell from the above screen and fell into his left eye. He tried washing the foreign matter out of his eye but was unsuccessful.
Employee was standing in water being treated by chemicals. Cause of this injury was attitude/holes in rubber boots. Miner proficiency, PPE, and company rules was a factor. Mining equipment/systems was not a factor.
EE was climbing on unit # 1230 dozer he slipped and caught himself with his left hand. He said his left arm was sore after. He worked until 7/10/13.
Injured was hanging feeder cable in the belt entry, bent over to grab feeder cable a rock fell out between rib line and roof screen striking him in the lower neck and shoulder. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was a factor, miner proficiency was a factor.
Employee lost his footing and fell onto a metal rib strap resulting in a cut that required 5 stitches to repair.
Injured employee was attempting to pull a rib strap out that was hanging loose with a pull rope when he felt pain in his left shoulder.
Employee was pulling belt and stacking it along the rib so a scoop could pick it up. He finished the shift and went to the doctor the next day.
Employee was riding in mantrip to section when he stated that he hit his hardhat on a roofbolt.
Miner was at home on Sun. 6/28/13 felt a numbness in leg & lower back went to Dr. the next week to find he has a ruptured disc. He called in sick & turned in for his 2 weeks vacation. He called safety director on 7/10 to report he thought he injured his back at work & was not sure exactly when injury occurred but he thought it could have been 6/26/13 when he was moving a gear box.
Employee was removing storage silo lid to measure contents and got lime in his left eye. Process was running to that silo making it positive in pressure forcing dust out the opening and under bottom of his safety glasses.
IE was driving toward section belt on Johnson Man-Trip. Riding w/right hand resting on warning bell & driving w/left hand on solid steering wheel. Man-trip hit uneven bottom causing operator to grip bell w/right hand. The bell bowl spun pinching his finger between the bell and the clapper causing a FX. Continuing to work full duty. Repair work was being done on a tail-pulley at conveyor #C-390; while handling the tail-pulley the employee slip in some mud and the tail-pulley struck his right ring finger causing a crushing and lacerations injuries with tendon damage.
Employee was working on belt line when he stepped in a hole and twisted left ankle.

Removal of track tensioner grease fitting with the track partially dislodged. The tension system had pressure in it. When the fitting was removed, the fitting struck employee in the nose.

Employee was preparing 8' section of 10"" pipe to be welded and installed. Employee unbolted upper portion of pipe to install purge valve. upper section of pipe came free began to slide off rack, employee grabbed for pipe and his left thumb for caught between pipe and angle iron of pipe rack.
Individual was hammering on metal segment on a CAT D8N dozer when a piece of metal broke and hit him in the neck. Just a cut at the time, later on at 1:30PM he notified his superintendent. When he went to the doctor after his shift 3:00PM and was found that a small piece about the size of a needle had to be surgically removed. This was done on 6/25/13.
Employee was driving a Volvo A40F truck when he hit a hole, jerked steering wheel hurting lower back and right leg.
While employee was bent over to disassemble T valve on waterline hose, the valve was stuck so employee pry it apart with hand tools. When employee stood up he felt sharp pain in his lower back. Reliance Medical Group released him to restricted duty and no underground work

Employee took the first step from the 5th floor to the 4th and his right knee popped. This resulted in pain on the inside of his right knee area. He continued to work up until he was taken off by a doctor on 8/6/13.

Employee was operating an end dump on the spoil dump when he went to dump the rock load the end dump traveled off the dump backwards overturning. The employee was taken to the hospital where he received 21 stitches total to the right forearm and left check area.
Tightening a bolt on the valve chest caught little finger behind metal tag when pulled finger out cut pinky finger.
Employee was operating a dozer on the clean coal pile. Dozer went into hole and jammed his back.
At approximately 09:55 am an employee was in the Crystal 290 heading preparing to survey. The employee was walking towards the face when loose material fell off the right rib towards the back striking the employee.
Employee states that when he bent over to pick up his gloves his knee twisted and locked up. He was unable to straighten it out.
Employee was dumping a hopper full of material, wasn't paying attention to his footing when he pushed the lever, he didn't realize his foot was underneath the hopper and when he pushed it downward the hopper fell forward and smashed his foot.
Employee was sitting when a rock fell and struck his neck/back. Went to doctor where he ordered him to remain off work for an uncertain period of time.
EE was hanging CM cable and sharp pain felt in right bicep.Scans revealed tear in tendon/muscle in right bicep.EE is going to physical therapy and surgery may be required.This report is filed late because individual whom normally filed 7000-1 reports was recently terminated and the person filing this report discovered on 7/15/2013 that it had not been reported.
RPI on 6/21/13 for low back pain after lifting a battery jack from a scoop. Was off work pending a CT scan on 06/28/13. Loverdige was notified on 7/3/13 of diagnosis.

Repairing cutting edge on excavator. Sledge hammer cracked & piece of hammer impaled left thigh.
Employee stated that he was pulling cable for the feeder move and injured his left arm. Started losing time on 1/2/2014.
Employee was stacking crib blocks. While doing so, he stepped on a broken concrete block, causing him to twist his right knee. This occurred on the last shift before summer vacation. While off, he went to a doctor and found out that he had a torn meniscus (right knee). He has been scheduled for surgery and should be able to return to work in a few weeks.
Employee was pulling on miner cable when he felt pain in his shoulder. Employee never missed work until his 7/11/13 shift.
A roof bolter operator was injured while preparing to install a roof bolt in the crosscut between the No6&No7 entries. operator started to position the roof bolter machine to install the roof bolt, the offside operator started around the back of machine. Offside operator was pinched between the coal rib and roof bolting machine. (MSHA immediately notified of accident)
Employee reports that he was traveling down an incline and something made his vehicle bounce. When the vehicle bounced, the employee's head made contact with the roof of the cab. He continued to work his entire shift without incident. He woke up several hours later while off shift and felt pain in his neck.
A rib bolter operator located on the left side of the miner was in need of a longer drill steel wrench and motioned to have the opposite side rib bolter to throw it over the top of the miner. When the employee threw the wrench over the miner he was unaware of the tubers position and the wrench struck him in the right temple.
Miner went into a sump area down from a shot crete area to advance a pump. Miner developed a rash and his skin showed signs of a chemical burn.
Employee stated that continuous miner cable that was hung on mine roof came loose and struck his Rt shoulder.
While replacing a brake chamber on a commercial truck, the employee unsafely attempted to disassemble the factory sealed portion of the brake chamber. The spring pressure discharged the brake chamber causing pieces of the spring to strike the employee over his left eye, causing a very deep laceration.
EE was bolting top when a corner rib draw rock fell hurting his back. EE finished his shift bolting top--He went to see a Dr. on 6.21.13 to be checked-contusion on back. The EE has RTW FULL DUTIES.
EE was pullling a wheel off of a Shuttle car using a slat bar and strained his lower back.

On 6/19/2013 employee had a first aid injury. Operator was coming down the ladder when his right foot slipped causing him to twist the right knee. On 1/9/2014 received notice that employee is having surgery on right knee with a causal relation to the first aid that occurred on 6/19/2013. Employee has been full duty up to surgery date of 1/16/2014.
Employee was moving mantrip into a side track and went to throw the switch back to the straight. When throwing the switch, the throw sprung back and struck the employee in the knee. The employee suffered a contusion to the right knee.
EE was walking on rocks and slipped falling down onto rocks breaking 5 ribs and his right elbow.
Employee was gluing PVC pipe, he finished one joint and was moving to another. Picked up tools, glue & primer can. Glue can was tipped, leaking on his coveralls & left leg. He realized this & proceeded to wipe the glue off his leg with a lit cigarette in his hand causing the glue to catch on fire. Burns to left leg, knee & hand.
The employee stated that while lifting the edge of a wooden pallet he stumbled backwards. In an effort to keep from falling, he reached out with his right arm to grab hold of the rib screen. He then felt a ""pop"" in his right shoulder. This event did not become reportable until 7-9-2013 when the employee began missing work.
Employee was operating the roof bolter when he placed his hand on the canopy then raised the boom smashing his hand between the roof and the canopy. He went to the hospital sustaining a fractured pinky.
993 loader was in the process of loading the 785 hauler when he approached the hauler with a bucket of material. His right side tire fell in deep mine break causing the bucket to hit the bed of the hauler the employee was in. This resulted in the hauler employee straining his neck/upper body. Was taken off work by doctor on 6/27/13.
EMPLOYEE WAS CLEANING THE ROCK BOX AT THE WASHING PLANT. THE BOX WAS PUSHED BACK INTO POSITION WHEN HIS LITTLE FINGER RIGHT HAND WAS CAUGHT IN BETWEEN THE BOX FRAME AND THE SHAKER SCREEN FRAME, CUTTING AND BREAKING THE FINGER.

While attempting to hang a length of water pipe from the mine roof the ladder used by the employee tipped over. The employee fell and landed on the ladder.
Employee was operating a walk through bolter and as he was installing a roof bolt he felt pain in his shoulder, while pushing the roof bolt into the drilled hole. Starting missing time on 1-2-14.
Employee was dragging a 6"" piece of water line when the other end of the water line dropped to the ground causing the end he was carrying to jerk out of his grip. The water line struck his SCSR in a downward motion causing increased stress load on his left knee causing it to buckle toward the middle.
On 6-18-13 EE claimed he slipped on loose coal straining his knee. He's been off work pending a medical evaluation. There was no witness to incident. There were implications this may not have been work related. On July 9 this claim was accepted as an industrial accident. Time difference between accident date and reporting date was pending investigation and acceptance.
EE was moving two rolls of curtain when he felt pain in his back. Employee never missed work until his 07/15/13 shift.
Injured employee was roof bolting on the left side of the continuous miner on the 1 North Mains section. He states he was struck by the ATRS pad causing him to be knocked into the machine resulting in a bruise to his right knee. This incident was not deemed reportable until 7/15/2013.
EE had finished installing roof bolts and was advancing the line curtain to provide ventilation to the face. He raised himself up off his knees and felt a tightness in his left knee. EE was administered first aid and transported to the surface where he was taken to the local hospital.
As Employee was traveling on main haul road (on his way home) he fell asleep causing him to wreck his truck. He received minor injuries to his upper body.
Employee was loading roof bolting supplies when he strained his lower back/left upper leg.
Stepping off roof bolter with right foot, left foot still on bolter. Stepped on lump of coal with right foot and sprained ankle.
Employee was installing outside bolt on first row of support when a rock fell around drill steel to the drill pod. While removing the rock from the drill steel a small piece of rock that had been on the canopy fell off striking his left index and middle fingers. This resulted in a fracture to the end of his index finger and 4 stitches.
Injured is now off work 8/16/2013. Scoop car was dragging plastic water line pipe, the pipe went sideways and hit right knee knocking him to the mine floor. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
Employee was in the procees of jacking up a loader when the machine fell. Employee sustained broken ribs and shoulder sprain.
Employee was hanging miner cable in the #5 Entry to clean and felt a pain in his back.
Employee was in the process of installing the lid on a VSI. Miners were using a telehandle to lower the VSI lid back into place. The lid was slightly misaligned. Miner used a pry bar to move lid into place. As he began to pry, the lid shifted and dropped into place and pinched the bar causing it to flip up and strike the left side of the miners face.
Contractor slipped and fell on a piece of belt causing the right knee to fold behind him. Contractor was treated at CAMC General Division where he was diagnosed with cartilage damage.
EE was setting up his work area to begin pumping grout. He had to join two pieces of pipe together. He was walking to where the poly was. There was a water pipe about 2 1/2' above the ground. EE leaned over and walked under the pipe. This is the point he said he injured his back.
I.E. was working on the fine refuse vibratory screen, he was climbing into work area he felt a ""pop"" in his left knee,and continued working. On 9/12/2013, the injured sought medical attention and was initially diagnosed with a strain of the left knee pending MRI results. On 10/3/2013 the injured was taken off work. His first missed shift was 10/7/2013.
Employee was using a 1/2"" drive ratchet with a 5/8"" socket to loosen bolt, bolt released unexpectedly causing employee to come in contact with a sharp corner edge of screen counter weight, thus cutting backside of right hand.
Employee was tightening the East Ridge dryer door with a wrench, he pulled back on the wrench and felt a pain in the left side of neck.

Aggravation of injury reported on 5/1/13. Employee twisted knee while installing silt fence aggravating his knee injury. Went to Dr on 7/17/13 who told him to take 2 days off work, then report to work with no restrictions. This report is submitted to add 2 missed days to the accident.
Employee was practicing CPR, bent down and pulled his back muscle
Employee was climbing down from vibration screen where he had unhooked welding lead that was hung up. He slipped and fell onto the concrete floor resulting in a fracture to his lower right leg.
Employee was operating loader #1044 in coal yard pushing up coal on the coal piles. When he backed down off a pile a large piece of coal got under his right front tire and he stopped. The chunk of coal burst jarring the loader and injuring his back. This is an aggravation of a problem he has had before. We were not notified of accident until 6/27/2013.
The employee was pushing a wheel barrow full of dirt/gyp and stepped wrong on his right foot causing it to twist.
Employee was working on mains belt cleaning up draw rock when he lifted a rock and felt pain in his groin area. Employee started missing work again 7/15/13, for this surgery
While walking through a belt drive construction area, the employee stumbled and fell, twisting his left knee. The incident became reportable when the employee under went surgical repair of the knee on 11-20-2013, and began missing work.
A rock rolled over striking side of bed he was operating, jarring him and causing neck pain.
EE was attempting to free a stuck bucket latch pin on skid steer 15-918. The extension that he placed on the latch handle slipped off and his right elbow was cut when it struck the loader frame.
Injured employee stated that while walking up the steps using a water hose to wash down portions of the rotary dump at the 6th floor he felt pain in his right knee. The injured employee finished the shift and worked the following day. He did not begin missing work due to the injury until 6/24/2013.
Employee was driving a mantrip when he turned to look back and unintentionally decelerated, when this happened a mantrip following behind hit the back of employees ride jolting employees back. At the time of the incident employee didn't take any medical treatment. Employee is now having pain and is seeking medical treatment.
Employee was stocking a spare tire into tire rack & overexerted his #3 finger on his right hand.
Injured employee was crossing the 1 South #2 belt at 104 block using the provided crossover when he slipped off the ladder and struck his left shoulder on the mine rib resulting in a bruise.
He was breaking rock & coal off the roof bolter cable. He was using a short roof bolt & a hammer. While doing so he missed the bolt striking his hand. He finished the shift and went to the Dr. the next day. He got a return to work slip for 6/18/13.

Employee was removing radiator cap when it blew off causing water and steam to come back on his left arm.
He was driving shuttle car from the continuous miner toward feeder, attempted to readjust himself and tires hit hole in roadway causing him to hit canopy with hardhat jamming his back. He continued working 2 weeks, went on vacation for 1 week and began missing work on 7-8-2013. Company conducted investigation to confirm validity and found it valid on 7-18-2013
As employee was loading belt tools into a mantrip, he twisted and felt pain in his lower right back. He has been diagnosed with a herniated disc in his lower back.
While walking from barge office to the barge bins, employee slipped in a muddy spot and his left arm extended up behind his head as he attempted to maintain his balance. (He did not fall). Employee experienced dull pain in his shoulder which was diagnosed in late August 2013 as a rotator cuff tear. Employee will undergo surgery for repair.
RBO was bolting in the face when he went outby to retrieve some additional curtain, while in the process of pulling up the curtain he stepped on a small rock and twisted his Left Ankle.
Welder was making a repair with some pre-cut pieces of steel. While lifting one of the pieces of steel into place the welder complained he felt a strain on his right side. His foreman sent him to the ER for evaluation.

Employee was assisting on the installation of track rail. One rail was jacked up and he had his hand under the rail to clean dirt from underneath of the rail. The rail rolled off the jacks smashing his hand. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
Employee was wedging stone, using a 60# wedge driver on 5' (22#) wedges, then pulling wedges out again. Does this repeatedly all day long. Felt pain in lower back, left side.
Employee was using an air moiler(jackhammer)-with the whipcheck in place when the fitting rotated & contacted the employee's fingers. X-rays were negative however, employee was given restrictions.
EE was getting out of his truck, lost his footing and fell of truck onto ground. He broke his left femur. EE is a licensed CDL truck driver that delivers finished product to customers.
Bolting in the #4 entry rock fell and hit EE on the head rock fell between bolts.
Driving a rock truck hit a hole in the road felt pain in lower back. Started missing work on 6/18/2013 and returned on 6/22/2013.
Employee states drove over something in road causing him to strike head on top of Ram car canopy. Employee was off with medical issues I didn't know he was off concerning the report he filed in June of 2013. Had surgery in Dec. 2013 concerning neck injury!
After checking a pump, Employee felt something hit his L arm. A witness said that as the dragline swung toward the pit after dumping, a rock fell from the lip of the bucket. Considering that the pump was located near the swing radius of the dragline, it is believed that centrifugal force caused the falling rock to hit the operator. Employee's L arm was cut and elbow was fractured.
Evidence at the scene suggests that the employee turned to dump and got his truck crooked an backed through the berm causing his truck to go over the embankment.
Employee exited PC-200 excavator, surface mine excavations machine, and was walking on level surface ground when he slipped and fell, stumbled on loose rock. Employee struck wrist on ground and fractured wrist.
Employee was operating water wagon and tractor employee slipped off of the seat of tractor and landed on his back.
Employee strained shoulder while assisting other employee with adjustment to boom on mobile crane. Conservative treatment was not successful and patient placed on restricted duty on 6/21/2013.
Employee was walking from his car to his equipment when he was going to work. He stepped on a rock with his right foot and twisted his right knee. Knee was sore and hurts to put pressure on it.
Employee was building a brattice on #1 unit when he bent over a trailer to pick up a block and felt something pull in his back.
Employee hit his head on a low pipe, and injured his neck. Received an X-Ray from chiropractor and treatment.
While bolting, a rock measuring 12 inches by 6 inches by 3 inches fell out and struck the employee in the right wrist. Employee stayed on the job until 7/11/2013 when his DR. gave him larger restrictions.
Employee was loading blocks into the scoop bucket. As he was walking toward the scoop carrying a block he tripped, causing him to fall and strike his left knee against the ground. This resulted in a laceration to the left knee that required sutures to repair.
Opening a bucket of plaster splashed out and got in his eyes. Employee did not start missing work till 6/13/2013 and returned to work on 6/15/2013.
Injured employee was pulling roof bolter cable around the block when he ran out of excess slack, as the cable pulled tight it forced the injured employee to the ground resulting in him striking his right shoulder causing a strained shoulder and neck muscles.

NOTE: This case was initially reported as a ""first aid"" case. Only weeks later was there an indication that employee asserted a work-related hernia. Description: Employee was attempting to open water valve. The valve was described as difficult to open and requiring exertion to open. Employee asserts he felt a small amount of pain in his groin area when opening.
Employee was moving Vent curtain and stepped in a hole twisting knee.
While trying to unplug a 6"" drain pipe for the washbay employee suffered a hernia. They were using a 1"" hose as a snake to flush out the 6"" line. The accident happened on 06/10/2013, he went in for surgery on 07/09/2013.
Getting pump car ready to move had finger in the top of door and door shut on finger
Employee was walking in the #3 entry of the 16I Longwall when he felt a pop in his left foot. He was diagnosed with a fractured bone in his ankle. Management is continuing to investigate this incident.
He was removing a draw bar pin when he pinched his left index finger between the pin and the locomotive causing a laceration.
Employee was operating the roof bolting machine and was in the process of moving his drill steel from the bolt tray to the drill pot, when a small piece of rock fell and struck his forearm. He was diagnosed with a contusion and soft tissue damage to his arm and placed off of work until July 7 or further examination.
Employee was hooking up 8"" hose to pump, he was moving the hose around when he felt something pull in his back.
Metal trailer tongue slipped off hitch, fingers were still on the torque, pinched fingers between tongue and crib block. Cause was not moving away after placing tongue on hitch. Compliance with company rules a factor, miner proficiency may be a factor, protective equipment not a factor, mining practices could be a factor.
Employee was installing roof bolts and as he was removing the drill steel rock fell hitting his canopy. A part of the rock that had fell broke off and flipped under the canopy striking his neck and back. Employee was immediately taken to hospital where he sustained contusions and abrasions.
Injured was disconnecting a water hose that has Cm644 chemical, which is a coal soap. When injured disconnect the hose the soap splashed into his face getting into his eyes. Employee finished shift and went to doctor that night said eyes was bothering him, and wanted to get eyes checked.
Employee was moving a continuous miner when he slipped straining his left knee. The employee continued working until he stopped to undergo knee surgery on July 12th. After the accident investigation was completed it was determined that the knee surgery was in fact due to the injury reported on June 10th.
Individual was operating a 20 ton motor pulling a supply train when the train came uncoupled from the motor. The safety cable did not break and the supply train struck the motor causing the individual to be jarred by the trains impact. Individual has pain in lower back and down to the right ankle.
Employee was installing 12' cable bolts when he felt a pain in his right shoulder.
Employee says he was digging a stuck boulder in muck pile. The loader bucket popped over the top of it when he curled it back causing loader to lurch forward hitting muck pile jarring employee. This caused a pain in his lower back and legs. MRI on 6/19/13 shows 2 herniated discs. Evaluation of injury and surgeons is taking place. Lost-time status effective 6/24/2013.

Operators were flushing out the Sodium Isobutyl Xhanthate header. A line split under pressure causing one operator to get reagent in their eye.
A loose bolt came loose from the dewatering vibrator and hit employee on the left thumb. Thumb was swollen.
Working on an engine compartment, employee felt debris enter right eye. Was evaluated by an ER physician on 6/8 and given a referral to eye doctor on 6/10. Employee didn't report for his 6/9 shift due to eye complications. He was evaluated by the eye doctor on 6/10; a foreign object was removed from eye, Rx drops were issued, & the doctor took him off of work for the date of 6/9.
Employee was assisting in long wall tear down and was retrieving the wedge setting tool when he was struck in the head by a 5 X 5 prop causing him to strike his right knee on the bottom
Employee was walking toward the last open crosscut to assist the continuous miner operator in moving from place to place. As he was crossing a coal hauler's cable, he tripped on the cable and fell onto the mine floor causing a fracture to his kneecap. He began missing time on 6-10-13.
Draw rock fell on top of the miner. Employee was getting it off with a rock bar and twisted his back.
After our investigation, it has been determined that the employee injured her knee while walking down a flight of stairs by the #16 ball mill.
Employee was at 1c tail kneeling down watching the belt on his left knee and when he went to stand up he pushed up with his right leg and felt his right knee pop. He was taken to the hospital where the X-rays did not show anything but was not released till he could get an MRI done on 6-14-2013.
While attempting to lift a 1250HP motor, using a forklift and a chain. The injured employee was holding the chain as the end loader operator was positioning the forks of the end loader under the chain. The left fork pinched his left little finger against the chain. Causing a laceration that required stitches to close
EMPLOYEE WAS GETTING OUT OF HIS RAM CAR AT HIS BATTERY STATION WHEN HE GOT HIS FOOT TANGLED IN A CABLE AND FELL FRACTURING HIS RIGHT TIBIA.
2 employees were dragging a pump, controller and power cable along the offside of the belt, Employee tripped and tried to catch himself. Within doing so he hurt his lower back.
According to associate, he injured his foot over a year ago and recently learned that it was fractured. The associate claims he injured his foot by kicking railroad car lids to open and close them.
Employee was traveling with an MSHA inspector. Employee was attempting to bend a sander bar when the bar slipped causing the employee to slip and fall. The lower left foot was became caught as the injured fell resulting in a broken leg.
IE was standing to side of a forklift when the operator of the forklift accidentally pushed on the gas instead of brake while IE was assisting in loading a part onto the forklift. Part of the forklift(Not the forks) struck the IE in the side, likely bruising him. Has not returned to work because of other findings by Dr.
Drill Helper was attaching alligator clips to 12 volt battery to power a diesel fuel transfer pump, when he did so he felt a shock and pain to his right arm.
Employee was attempting to change a roller on No 2 belt as he crossed over the belt his boot got caught on the belt line rail causing him to jump off the belt. It sprain his foot.
Injury not reported until 06/11/2013 at 11:15AM Highway Truck driver possible sprain to stomach area while opening hood on truck. Supervisor was present at the time, no indication given as to supervisor by the driver of any injury at that time. Injury reported as noted.
Pushed clutch down heard a pop in left knee. When he could stop he got out of the truck and could not put weight on left leg.
EE was sitting near a section power center when a 12,470 high voltage had a short circuit and blew off striking him in the elbow.
Employee was assisting another employee to install fire suppression in a 12 foot high area. Stone fell from the corner of the roof and struck the employee on his hard hat.
Changing a wet brake system on a scoop, picked the wet brake up, felt pain in lower back and legs. Started missing on 7/23/2013
Employee was sorting through a box of paperwork when a piece of paper fell on the ground the employee bent over to pick it up and his back went out causing him to fall forward and hit his head on the door. Employee was sent to the doctor for evaluation. No damage or impairment was caused.
Adjusting conveyor and a 4"" PVC dust line(full of dust) broke from overhead from a distance of 3'. He was hit initially on the hardhat, then in the back of the neck and on the side of the rib cage (letf side)
Employee was attempting to move around the end of a rail car, when he slipped and fell striking his head on the rail car. Height of fall was approx.3 ft. Sutures required to close wound.
Picked up roof bolts out of scoop bucket turned towards roof bolter and legs gave out hurt his back.
Employee was hooking up a charger receptacle after making a circuit breaker test. When he pulled on the cable to hook it up he felt a pain in his groin. He returned to work on June 6 and 7. He took vacation June 10 - 14. On June 17 he had surgery. He missed June 17 - 21. The mine was idle from June 22 to July 7. He missed July 8 and returned to work July 9, 2013
Employee was cutting off the right side of the truck bed. Had already removed 2/3 of right side. Cut 3 of the 4 wearbars/gussetts. Employee then moved back to the outside of the bed to continue cutting when the port he was working on broke loose and fell striking employee and pinning him on the right side leg and arm along with his left foot.
Injured employee was stepping through a mandoor at 10 Left when he twisted his left ankle causing a sprain.
Truck was loaded with dirt and rock, leaving the pit area and came through a swag in the road. Rock shifted to left tail shoot causing the bed to rise and throwing the front end of the truck up. Truck bounced on tires and box slammed down shattering passenger side window.
Employee was bringing a diesel part to the underground diesel shop when he tripped over a cable causing him to hit his left arm on a table. Employee did not start missing time from this accident until 06/13/2013.
Roof bolting @ the face, bottom was muddy and wet. Leg from knee down to foot were sucked into the mud. He twisted his upper body and his knee popped.
Employee was working on track hoe, trying to remove the pin from the cylinder that operates the bucket. While pushing the pin out, the bucket slipped and the employee tried to catch, catching his right hand between two pieces of metal. Right hand suffered broken bones and skin lacerations.

Exploration drill helper moving a pipe back to the rack stand. Helper did not use the proper method to handle the pipe twisted and turned instead of lifting and walking with the pipe. The helper has experienced severe back pain and will be evaluated medically before return to work.
The injured states he was putting nozzle back on the fuel truck. It was kinked the swivel was not swiveling right which caused the swivel to twist and caused injury to the injured's right hand.

EMPLOYEE WAS STRUCK ON THE LEFT THIGH BY A 40 TO 50 LB. ROCK THAT FELL FROM ROOF. HE DEVELOPED A BRUISE ON HIS KNEE THAT WOULD NOT HEAL BUT CONTINUED WORKING. ON OR AROUND 06/24/2013 HE HAD TO HAVE MINOR SURGERY ON HIS THIGH TO DRAIN A HEMATOMA. HE WAS PUT OFF WORK TO RECOVER, THUS BECOMING A REPORTABLE INCIDENT.
While helping to lift a wooden crate, a board came loose. He lost his balance and fell to the ground on left wrist.
EE hit his hardhat/head on mine roof strap hurting his neck.
Employee helped remove impeller from main pump. He stood up straight and complained of back pain. Worked remainder of day. Went to ER that night after pain increased. X-rays confirmed pulled muscle. Out until Thursday, June 06.
Employee tripped on a track tie and felt pain in his right knee. He continued to work all scheduled shifts until 7/3/2013.

Employee was stepping down from Hitachi Excavator by use of the ladder. He stepped onto the ground into a hole/uneven surface and fell down catching himself with his left arm causing contusion and pain to left shoulder.
Full drums of coal had to be emptied into stockpiles. Pulling full drums to dump out coal pulled muscle.
Employee was diagnosed with chronic back issues and doctor has a no work restriction.
Employee stated while welding, he was having trouble with the ground and when he lifted his welding hood the welding rod arced causing flash burn to his eyes.
Injured employee was walking from the scoop charger to the power center on 7 Right when he stepped on a nail that punctured his boot and right foot. Employee finished the shift and took one floating vacation day following the incident.
While installing a discharge lip liner on the bottom deck of K-4 he twisted his back.
Employee saw the man trip but thought there was enough room to travel by on a 3-wheel ride beside of the man trip. He stated that he went through the fly and had his left foot lying sideways sticking out over the edge of the 3-wheeler. He got too close to the man trip under the fender wall where the wheel is located on man trip injuring his left foot.
Employee was in parking lot for dozers when he thought he felt a shock. At the time he did think he was hit by lightning. He went home and worked the next day June 2, was off June 3 and 4, back to regular work on June 5 2013.
Employee was moving leaky feeder cable up. When he crossed a mobile bridge he said his knees buckled when he put them on the bottom. Also he said they had been hurting for a couple of weeks
Operator of the 508 haul truck states that she hit a pot hole.
EE alleges that while operating a backhoe scaling quarry highwall, his lower back began to hurt after being jarred when a large rock hit the bucket.
Finger was fractured when a metal door was closed on a Millwright's hand.
Installing engine in D10T, it had been raining all day & was installing rear mounts, drive line & hoses in hole & standing on angled belly pan & slipping & right knee popped and overextended.
Employee was preparing process samples for QC testing on June 27. Turned his body to step and his foot stayed planted while knee and body turned. Floor has rubber, cushion mat that may have contributed. Report only, knee bothering him on 7/2 and we took him to clinic. Possible torn meniscus in right knee. Follow up MRI scheduled for July 11th.
The employee was removing a welder from the hitch of a pickup truck, when he sat welder down the jack stand wheel folded up and the welder struck the top of the employees left foot resulting in a hair line fracture.
While switching seats in his shuttle car the employee felt a pain in his left knee.
Two employees were working on a crooked chute. One employee was using a come along to help straighten it out. The injured party put his left hand down on the railing to reposition himself, and at that time the chute let go and rolled onto his hand, ripping and smashing the baby finger on his left hand.
EE was over seeing repair work to roof bolter, while helping remove lid rock fell from between roof support striking him on foot.
Employee turned off the agricultural lime plant to dislodge a rock in the tail pulley. After locking of the conveyor he dislodged the rock. When he did that, gravity moved the conveyor belt entangling employee's arm in the tail pulley.
Employee was descending a ladder when he missed the last rung and was suspended by his right arm. EE was taken to onsite medical facility. EE was placed on restricted duty on 06/27/13.
Employee reported right knee swelling and pain after repeated entry and exit from a tire handler. He was evaluated by a health care provider on 7/9 and released with work restrictions that prevent him from working within his normal job scope.
Employee stated back had been hurting. He jumped off the running boards of the bulk truck. Felt a tear in his back which totally immobilized him. Ambulance to emergency room. Emergency room doctor treated him for back pain, sent him home with medications and told him to see his doctor. Employee couldn't get an appointment until 7/1/2013.
Employee was lifting the cover off of a measuring port on top of a material silo in order to measure the level of the silo. The employee bent over to lift the cover and strained their lower left back.
The operator of the #4 water truck was climbing back onto the first rung when her foot slipped and her right knee popped.
Employee was rolling an empty 55 gallon drum onto a loader bucket to put in the scrap metal dumpster. His glove stuck on the drum pulling him forward and injuring his Right Shoulder
WHILE PICKING MUD & ROCKS OFF OF CONVEYOR BELT, HE REACHED OUT TO PUSH A ROCK OFF THAT WAS HEAVIER THAN EXPECTED. THIS SPRAINED HIS RIGHT SHOULDER & HE NOTIFIED FELLOW EMPLOYEES & MANAGEMENT IMMEDIATELY. HE CHOSE NOT TO SEEK ANY MEDICAL ATTENTION UNTIL 07-12-2013 AFTER CONTINUED SORENESS PERSISTED.
The employee and his partner were bolting #9 entry at cross cut #94 on #1 unit. After moving forward the atrs against the mine roof. When doing so the atrs loosened a rock in the roof. The employee on the opposite operator moved to his drill station to swing his boom out and the piece of rock fell out on his left ankle. The rock measured 4' long X3' wide X 4"" thick.
Employee was on scoop unplugging the batteries when he lost his balance and fell off the scoop. He caught his right knee in the fender well resulting in a meniscus tear.
Employee was pulling a water hose and felt a pain in his belly button area. Doctor placed employee on restrictive duty on 06/21/13 and is on restrictive duty until his surgery is scheduled.
EE was trying to lift a shim on a thrust roll and felt a pop in his right arm. EE did not think he was injured or request medical evaluation until 07/02/13. At that time he was diagnosed with a ""strain"", returned to work and scheduled for follow-up. On 07/24/13 he was diagnosed with Other tenosynovitis of right hand and wrist and given restrictions affecting his duties.
Employee was manually turning crusher sheave, when sheave slid off crusher shaft and pinched 2 fingers between shaft and sheave upper flange. 1 finger broken and 1 finger dislocated on right hand of employee
Hauling stone from pit to Primary Crusher, Truck went over highwall.
Employee said that on 6/5/2013 while replacing a bucket elevator he injured his back but did not report it. Employee said while shoveling coal on 6/17/13 from under a conveyor, his back popped again and was hurting. He went home at this time.
Employee slipped climbing over rail and strained bicep.
EE was hosing down the floor in the #12 Mill Sump and slurry dripped from above, into the ee's safety glasses and started to irritated an eye.
Employee was refeeding into the grizzly and was bending over to pick up a 50lbs paper bag. When he reached down to pick it up he felt a pop in his back and it began to hurt. He continued to refeed into the grizzly and the pain continued in his back. He was taken to Pershing General Hospital where he was treated for a back strain.
Employee's left thumb was caught in a 1 ton air chain hoist - lifting side. Hoist was not de-energized when the employee attempted to move the hoist. Hoist was accidentally activated, which caused employee's thumb to get caught in the hoist mechanism.
Employee was placing 20 lb. wooden blocks next to the railroad tracks in an awkward position with his hands extended out in front of him to re-rail the front trucks of the locomotive. He overextended and when he stood up felt pain in his abdomen. The employee is on restricted work duty.
While attempting to clear a blockage in a discharge line the employee fell to the same level injuring his right shoulder. Employee did not request medical attention until 7/30/13. Employee was schedule for MRI on 8/28/13 and had surgery on 9/10/13. Employee performed normal work duties until 9/10/13. Lost time began on 9/11/13. Torn rotator cup.
Employee lifted pit truck hood and felt back pain in his back. He continued working for approximately one month at this time he went to the doctor on 07/16/2013. The doctor put employee on restricted work duty at this time. Because of the restricted work duty, we have been unable to accommodate this employee. He is still currently off of work.
Employee was tightening bolts on machine. He bumped his shoulder & lost his balance & fell, lending on both feet. He was diagnosed with a heel fracture.
The rubber tire operator was hit with a piece of fly rock from a blown haul truck tire.
Employee was assigned to cut aprons off the Rap Screen using a torch. He had unrolled the torch from the truck and hung it so he could reach it. He squatted on the ground to begin cutting metal off the screen when he felt a pop in his right knee.
While setting steel, a 10' piece of 3/8"" angle iron fell three feet vertically onto the miners right foot. The steel punctured the miners right boot and also the skin on his foot. His status is Full Restriction, stay off feet and keep foot elevated. Possible small fracture and infection.
during a belt move, was lifting a roller up to move the bed rail when a post fell out and struck his left arm (fracture)
Employee felt a ""pop"" in his lower back while lifting a 20 pound box. He was evaluated by a physician and physical therapy was prescribed. He attended his first session of physical therapy on 7/18 and his second on 7/23.
A flat car was being hooked up when the employees right hand got pinched between the motor and flat car.
EE was carrying a power cable pot head when his left knee gave out
Contractor employee was on a ladder helping place a strap around a clam-shell bucket. While attempting to grab the strap he fell from the ladder and broke his right ankle.
While drilling at the face of 690, a rock rolled out of the face, Miner jumped back to avoid rock and felt his knee pop. Miner took ibuprofen and finished shift. When miner got home knee started to swell.
EE was walking to slope ride when he slipped and fell. He tried to catch his fall using his right hand. Fracture to right hand.
EE was working on the bolter when the chain ratchet slipped dropping the scissors (arm Pot) on the left index finger causing a laceration that required sutures. EE missed one day to follow up with the doctor and has RTW modified duty until the sutures are out and laceration healed.
Employee had performed preventive maintenance on 80' Power Screen Stacker. Employee was removing debris from the Stacker Belt with compressed air. Employee instructed assistant to start Stacker motor and engage belt. Stacker Belt caught air wand pulling employees arm into belt and roller resulting in injury.
He had been standing all day running the plant when he turned wrong & hurt his back. He went to the Dr. on 6/10/13 the Dr. said it was a strain and employee went back to work. He was still having some trouble 7/11/13 they scheduled an MRI. He seen surgeon on 7/22/13 & decided he didn't want surgery. He then went back on 8/8/13 & he decided to have the surgery on 8/28/13.
Employee was pulling on a chain to open a valve and felt a strain in his groin. He was evaluated and diagnosed with a hernia. His work duties were restricted on 6/11/13. He is scheduled for surgery to repair the hernia on July 2, 2013.
He was hitting a chisel with a hammer, missed the chisel and hit his thumb. Fractured his left thumb.
As an employee was walking down the Gold Hill heap side-slope, she lost her footing. In an attempt to break her fall, she extended her arm and landed on her out-stretched right hand. The employee was transported to Nye Regional Medical Center where it was determined that her right wrist was broken.
While guiding a detonation line through an emulsion hose, the feeder hose became dislodged and fell under its own weight. The whipping hazard was recognized and the employee ran away from the line, tweaking his lower back in the process.
Employee was removing rock from behind the conveyor belt when power was turned on. His right arm was caught by the belt and broken.
Employee was walking across blast pattern and rolled his ankle.

While lifting a grid cover from a water truck, employee felt a ""pop"" in his left bicep. He was evaluated by a physician and released back to work with restrictions. Surgery has been scheduled for 6/17/13.
Employee slipped on a small rock while walking and sustained a slight injury to their left hamstring.
While throwing a rock from the crusher the employee struck his finger on a piece of metal.
Installing 2x4 light fixture in three offices. Shut off motion light switch button and placed tag over light switch. Then made sure no voltage on both lights and took one fixtures down. Installed new light hooked up ground wire first then second wire hot to light disconnect shock hit was approximately 2 ft. on ladder and fell off ladder. Employee taken to UAMS for observation.
While putting away 1/4"" screens for 3&4 bottom decks, employee was on one end of the screen bending down and co-worker on the other. Employee, in the process of standing up, tripped on the wood that is used to keep screens off the ground. As he started to fall backwards, he caught himself before falling to the ground. In the process, he twisted his back
Employee was replacing a gear drive on conveyor. While removing it, he twisted wrong and hurt his back.
Employee states as he was installing roofbolts, his left hand was struck by a piece of rock from the mine roof. Lost time as of 6/6/2013
This is a re-occurrence and further treatment of an old injury. There is no new incident/injury event. Original MSHA 7000-1 was closed on 03/08/1995. EE successfully petitioned Worker's Comp to have his claim re-opened and as of 04JUN13 he is receiving further medical treatment for the original injury.
Employee was driving the company pickup and was crossing a RR track but failed to notice that the sand normally between the rails had been cleaned out leaving a 5"" drop. Even though the employee was wearing his seat belt and hard hat he bounced up into the cab roof and injured his neck. Lack of visibility was the main contribution to the incident.
Miner received cement burns to right ankle when cement saturated water leaked through a hole in his boot.
Employee was descending the ladder on a mobile loader, tripped, and fell approximately 4 feet to the ground. One of the steps on the ladder had broken and caught the employees foot when he stepped on it.
Employee was rolling up a spool of Leaky Feeder Cable on a cart in #4 entry, 1st Right Intake when the cart flipped and he tried to catch it as it was falling and felt pain in his back.
While removing drill steel from magnet, finger was pinched between magnet and drill steel.

Employee was pushing an oversize tire over a gate as two coworkers were pulling on tire when it rolled backwards and tweaked his right wrist.
EE was stacking 50 lb. bags of material onto pallets when he felt a pain in his right wrist. His wrist became swollen and painful to move - EE's physician has prescribed a brace for his hand and restricted EE from using right hand while working.
On 6/30/13 employee reported a sore back from getting in and out of a service truck. Employee was evaluated by a physician on 7/1/13 and given restrictions. He was released to full duty work as of 7/8/13. Please note: This report is being submitted at this time to terminate a citation issued during a Part 50 audit.
EE attempted to take a step to begin walking and stepped on a rock forcing her knee backwards.
Descending a two step ladder that crosses over a trough and he stepped on a large piece of hydrate and twisted his ankle.
Employee was washing a press and the pressure of the water hose knocked him off balance and he struck side of the his back against the press causing a contusion.
Dismounting the portable crusher ee missed a step causing him to apply all his weight on the right leg/knee.
Employee slipped and struck his knee causing a fracture.
Employee was ascending stairs to control cab of gantry crane and started to have low back pain. No slip trip or fall at the time.
Operator was leaving the control room headed to ground floor. He did not have proper footing on last step, causing him to slip. He then grabbed the handrail with his left hand and felt a burning sensation in his left bicep.
While in the process of removing a wedged bag from the packer line EE picked up the bag and felt a pop in his left upper arm. He has torn his bicep.
The employee was removing the wear package on a CAT 994 Loader bucket. The employee was attempting to loosen a lip protector striking it with a mild steel dead blow hammer. A small fragment of metal from the edge of the lip protector stuck the employee in the back of his left lower thigh. The employee was placed on light duty to assist healing.
Employee was replacing a hydraulic cylinder when the cylinder slipped and fell pinning his middle finger on his left hand between cylinder and metal step.
Left foot slipped on granite gravel, fell hitting & cutting right forearm on a metal bumper
Associate was on one side of the anfo truck prepping the primers when another associate on the other side of the anfo truck let down the outriggers onto the first associate's foot breaking 3 bones.
Employee tripped over an angle iron that was sticking out of a dirt pile. Was not reportable at the time, but the employee has since required medical attention.
Maintenance crew ee and I were attempting to pull the counter weight off the timing side of a Telsmith screen shaker. We had a puller and two come a longs on it as well as heating it. it started coming off as we cranked on the come a long when all of a sudden it broke loose and flew off resulting in smashing his hand between the counter weight and I beam.
Employee was preparing pallets for shipment in the #1 dock area. The forklift operator was moving pallets in the area. As the forklift operator was backing up, the forklift struck the effected employee in the left leg and ankle, resulting in a non-dislocated hairline fracture to the left tibia. Employee was treated by a doctor and returned to work.
Employee was walking over to assist another employee when he slipped on slick bottom and fell hitting his right hand on the mine floor. He visited a doctors office the next morning and was referred to another physician at a later date to be determined for follow up. The employee returned to work on his next scheduled shift on 06/27/2013.
Employee attempted to move a piece of rail road track that was laying on another piece of rail road track, by himself, when it moved, it rolled over and smashed his finger. He got stitches and they surgically repaired the right hand ring finger.
The Employee was rigging a steel I-beam and the beam slid off of the dunnage and fell on the Employee's Left Foot causing a contusion.
Employee was hitting the chute between 7A and 10A conveyors with a sledgehammer in order to loosen choked material. As he was swinging the sledgehammer, he felt pain in his back.
Heavy Mechanic employee was removing the rod end pin from a production drill utilizing a manufactured drift from Caterpillar. The employee became fatigued from multiple strikes with 5# hammer which resulted in a mis-strike, hitting his hand.
Employee states he was installing flashing on a conveyor belt to prevent spillage. As he was pulling on the flashing to properly place it his right hand went across a piece of jagged metal resulting in a laceration to his index finger on his right hand.
EE was walking past a co-worker when the co-worker decided to toss a piece of waste stone into a waste hopper which contacted EE on top lower R-forearm causing laceration requiring multiple sutures. EE medical follow up requires surgery on tendon scheduled for 07/02/2013.
Walking on an uneven surface while performing a pre-task inspection, left knee hyper-extended as he strode and hit a depression.
Employee was assisting with installation of air conditioner and strained shoulder while using a come along to raise unit into place. Patient was placed on restricted duty today 6/28/2013
While removing the shiv from a 300 hp motor that assist in running a Hazemag Crusher the shiv fell off of the shaft smashing employees fingers. Employee had to have his right hand index finger stitched and his second finger splinted.
Electrician was opening electrical cabinet door when hinges broke causing the door to fall toward him. As he caught the falling 5 foot by 3 foot door, it twisted back causing low back pain which required medical that afternoon. The electrical cabinet was located in the alternative fuels area of mill at ground level.
Employee was welding and fell. Employee fell on 6/24/13, he didn't think he was injured and chose not to seek medical aide. On 6/26/13 he reported to his supervisor that he felt that he had damaged his ribs in the fall. There is no lost days - employee is on light duty.
Employee was using an impact pun to remove nuts from studs on vessel piping. Stud spun & gun backed up pinning Rt thumb between gun & bolt on flange. Resulting in small cuts to thumb & some swelling. EE iced & returned to work. EE sought medical treatment on 7/15/13. Xray showed non displaced fracture. Restricted duty 7/15/13
EE STATES WHILE TIGHTENING LUG NUTS ON THE TOP OF A WHEEL WHILE STANDING ON THE GROUND AND HOLDING THE AIR GUN ABOVE HIS HEAD HE FELT HIS BACK POP BETWEEN HIS SHOULDER BLADES AND THE BASE OF HIS NECK. NO PAIN INITIALLY PAIN STARTED AFTER HE STOPPED MOVING AND WAS SITTING IN TRUCK
The employee was completing a blow down process on a press when he jumped over some process lines and sprained his knee
Employee was climbing down off equipment and strained groin muscle.
Employee was raising the electric boarding ladder on a haul truck when her arm was pinched between the ladder and the body of the truck.
Employee was helping to move a steel plate when the plate fell on his left foot, landing just behind the steel toe. He suffered a cut and bruise on the top of his foot.
Step off 6"" concrete pad, twisted ankle.
The maintenance employee was checking the wet process float cells. The employee climbed up the feed end of the float cell bank and reached up and grabbed a small guard at the top of the cells. He got his ring finger caught in the pulley on the backside of the pulley causing a cut and a fracture of the end of the finger.
Employee was working on running, unguarded equipment and employee moved hand and fan stuck left index finger
Vac truck had a clogged vac hose from a rock. Employee was trying to relieve the blockage and was using a box knife to cut hose to get rock out. Knife slipped out of hose and cut employee on the right wrist (3/4"" long cut).
Employee was moving 4 schedule 40 carbon steel pipe (35lbs) into position at a receiver tank. The employee he felt pain in his stomach but he thought it was something he ate. The employee reported his injury the next day via phone (Sunday 06/23/13) and was taken to the doctor on Monday for examination on Monday 06/24/13 for examination and was diagnosed a Umbilical hernia.
While scaling down a piece of stone, the employee was struck on the right foot by a piece that he did not see falling. This resulted in a fracture to his right little toe.
Injured right tendon in calf closing gate.
Employee was attempting to park the Kawasaki Mule at end of shift. Employee was turning the Mule as the pathway, which was flat, changed from gravel to asphalt and the Mule turned over on its side. Injuries were scraped left forearm, broken finger tips to left ring finger and left middle finger. Left palm required 3 stitches and the left index finger required 6 stitches.
Ind was moving a bolter head out of a scoop bucket when he got it out of the bucket he felt a pain in his back and continued to work and seek medical attention on 7/10/2013
Employee fell off the steps of a service truck. Thought he was nearing the ground and missed the last step. He fell backwards and caught himself with his hand. Has a contusion on his left hand
The operator of the 646 haul truck stepped into the parking ditch and when he planted his left foot, his left knee popped.
While walking on the drill pattern, employee stepped on a small rock which caused him to roll his left ankle and fall on his left knee. This resulted in a sprain, requiring 2 days of restricted duty.

Employee was entering skid steer when turning around to sit down he strained his lower back.
Employee was removing wear plates from the tub of dragline. The plate fell from approximately 30"" and struck the employee's right leg, causing lacerations to his leg. He received sutures to close the wounds, prescription medication and he is scheduled with an Orthopaedic Surgeon to follow up on the internal lacerations to the knee.
Employee stepped off of mantrip and twisted his left ankle which resulted in a severe sprain. The employee was placed on restricted duty.
Slipped and fell from a loader platform. He caught himself with his right arm on the handrail, pulling ligaments in his shoulder.
Employee was removing a grate over a sump in order to clean the sump. Employee used a 1/4 inch round stock hook approx. 3 feet long with a 1 inch 90 degree bend on the end of the hook. The grate slipped off the hook and struck the employees right foot just behind the steel toe of his boot resulting in a fracture to his Medial Metatarsal of his right foot.
Employee removed kiln camera. Carried over left shoulder and walked down from burner floor to card. Drove cart to electrical shop. When employee picked up camera from cart, noticed lower left back was hurting.
Employee was helping to change head light on skid steer. Operator of skid steer moved forward to position light and pushed throttle forward with his body and ran over employees foot.
Employee felt discomfort to the right shoulder and bicep area when lifting & disposing of a wear cap weighing 79 lbs.
Employee was installing new V-belts on K-3 shuttle drive using a pipe wrench to roll on belts when the pipe wrench slipped smashing his left index finger between the wrench and motor base. Employee suffered an open wound and hematoma.

Employee was prying on rock in oversize chute with a bar and strained his chest muscle.

Technician was installing hydraulic tank onto 14M motor grader in the rain, slipped and struck tail bone on ripper frame causing contusion
Employee experienced back pain after his trainee backed into the dipper and hit a pot hole.
Employee was working on the engine of a haul truck & was leaning on his right side stomach to reach across the engine. He felt discomfort in his right side stomach. Employee was not seen by a doctor until 6/18/2013.
While replacing guard after changing belts on screen the guard slipped and caught his hand between the guard and motor sheave resulting in a laceration to the hand.
Mechanic was removing the dipstick for the transmission on the 52M Broderson crane, when his hand made contact with the fan blade.
Operator was backing down onto the location in a dump truck to be loaded by an excavator. The truck ran over a high point in the road which turned out to be a piece of ore and the truck bounced and jerked from side to side, the seat bottomed out causing him to strain his lower back.
Employee strained his back while carrying grating and was released to full duty at his initial visit. At one of his follow-ups, 2 weeks later, the employee's work status was changed to restricted duty.
EE was using a battery powered drill and it caught and twisted his arm causing discomfort in his elbow.
Emp. was operating a Skid Steer moving material near the belts when the skid steer went forward into a hole. The equip went forward into the hole & then righted itself up with jerks until it stopped & rested correctly on the ground. Everything was fine until the employee got off the piece of equip 20 minutes later when he began to experience pain in his back.
Employee was unloading bags of ANFO out of the Buckley trailer and into trailer 901. As he picked up a bag on the pallet, he twisted at the waist and felt pain in his lower back.
Employee was bagging M35 100lb bags, he threw a bag on the pallet and felt a pull in left forearm, resulting in muscle strain.
Co-worker was using a hammer to drive the pin out of the excavator tracks employee was on the other side when the pin came out and struck his arm. This caused a large cut on employee's right forearm. He returned to work the following day.
Process Operator was using a wrench to loosen bolt, felt a pop in lower back.
Employee was operating a 980 front end loader on a Barge to cleanup the deck of residual limestone. The loader bucket caught the edge of the barge and jarring the loader operator.
IE was installing conduit in a 18"" trench for temporary power to a contractor's job trailer when he went to step out of trench he felt pain in his right lower calf. IE was transported to medical facility where he was diagnosed with a muscle strain and partial tendon rupture. IE was prescribed a pain reliever and given restrictions to not climb ladders.
Employee pinched finger while attempting to remove a large rock from 613 dozer push arm.
The employee was bending a 6ft. roof bolt using the ATRS and his hand slipped causing his left arm to strike the bolt plate resulting in a laceration requiring 3 stitches.
Employee was removing a cutter bit from the longwall shearer, and a piece of metal debris entered his left eye.
Employee was using the man lift. Employee mis-stepped with his right foot and fell backwards toward the ground. His lanyard was still attached to the manlift and kept his upper body from falling all the way to the ground. However, his right foot and leg were on the ground and his left leg was still on the channel iron. At this time employee felt pain in his left knee.
Employee worked 6/14 and 6/15 with no report of any injury. Was a no call/no show for work on Monday 6/17/13. Reported to work on 6/18/13 and said he might have hurt his back on Friday while lifting drill bits into pick up truck with another employee.
Employee was using a razor knife to trim a plastic swab stick to fit a piece of tubing to be cleaned. The knife slipped and lacerated the thumb and index finger.
Employee was operating a loader, as he pushed the loader bucket into the stockpile, he stopped. During that time a rock rolled from under the bucket to under the left front wheel. As the operator backed away from the stockpile the left front tire drove over the rock, causing the tire to slam down on the ground.
Pulling hydraulic assembly out of backhoe when it allegedly slipped out of hand and dropped on finger.
While removing belt structure from the longwall belt line he caught his right middle finger between two cradles lacerating and fracturing the tip of it. The cut required four sutures and the fracture a splint. He returned to work in a modified capacity his next scheduled shift. THIS IS BEING REPORTED AS A RESTRICTED WORK INJURY.
The miner was helping change screens on a screen plant. He was standing on the discharge belt and working outside of the catwalk guardrail. He stepped backwards off the discharge belt and fell approximately 3' onto a shuttle belt.
Fell down the wooden stairway at R84O trailer. There were 3 steps that the employee fell down. He contused his left knee.
The employee was removing a 1/2"" piece of plate steel when it slid unexpectedly down pinching his right index finger between the plate and the edge of the shaft.
Injured and a second employee were in the process of changing L443 Weigh Belt. Injured was holding up the tail pulley take-up as employee 2 unscrewed the take-up bolts. When the bolts were removed the tail pulley dropped farther than anticipated (10""-12"") to the housing below. Injured' s finger was pinched causing a laceration.

Miner was striking a chisel with a 3lb hammer to split limestone. As he hit the chisel he noticed his hand bleeding and a piece of the chisel had cut and embedded in his right hand.

Employee was putting a battery man trip on charge when he turned around and caught his ear on a fire suppression sprinkler head, causing a laceration.
Employee was getting out of the ram car and twisted his knee.
Employee was leveling belt structure. He felt pain in right elbow to hand while using bumper jack.
Employee was on a ladder using a welder on a metal frame. The frame suddenly broke loose and then struck the ladder causing the employee to fall approx. 4 feet.
Employee was helping with a belt move and a piece of belt structure fell and struck employee on hand.
While backing up a D10T dozer, ran over large rock and jarred/strained neck area. Employee was unaware that a rock fell out of berm while pushing material.
Employee was moving a water discharge hose when he tripped and fell to the ground. He struck his face on the ground which caused a cut above his right eye which required sutures.
Employee running excavator, swarm of bees came into cab, was stung by bee, jumped out of cab, landed on feet and twisted right knee.
Employee was moving some chains and was apparently bitten by an insect. Area became inflamed and he was seen by a physician on 6/12.
Worker was splitting stone using a hammer and chisel when he felt a pain in his Left wrist. Reported the injury and was treated with ice. 06/11/2013 reported for work and requested to see a Dr. went to clinic. (sprained wrist) restricted, NO LOST TIME!
Lifting out scrubber roller, roller shifted to caught finger.
Pipe wrench slipped off the stuck roller on the chain. Employee jerked back and felt a pop and instant pain.
Two electricians were installing new fluorescent lighting into suspended ceiling gridwork. The injured worker was standing on the floor after handing a light assembly to the second electrician who was on a stepladder. In trying to maneuver the assembly, the light fell. A corner of the light struck the injured employee and caused a laceration of the right, inside forearm.
During a severe thunderstorm a miner was closing the shop door. The wind caught the door and slammed it in to his shoulder.
Employee was cleaning the line from stage #4 to the riser and as he cleaned out a plug, material rushed out of the line and got on his hand. Employee initially refused medical attention until 6/13/13.
The employee picked up the metal rolling pin to begin rolling samples when it slipped out of her hand falling and landing on the top of her left foot.
While lifting a rail out of a product screen, he felt a pop in his back.
