Accident type, without injuries

At approximately 8:00pm a roof fall was reported on the 7 West Belt at 25 1/2 crosscut. The fall was approximately 50 feet long by 16 feet wide and 8 feet high. No other part 50 criteria applies.

An alleged face ignition occurred on the 3W CM section while mining at the face of the #3 entry. Investigation revealed a missing spray in the bottom center spray block of the CM, which caused the entire 6 spray block to lose water pressure. There were no injuries or damage to equipment or mine structures as a result of this event.

At approximately 10:43am, a roof fall was reported on the 8 South #2 Belt from 2-32 1/4 to 33 crosscut. The fall was approximately 40 feet long and 8 feet wide and it varied from 8 to 10 feet high. No other part 50 criteria applies.

At approximately 10:36 a.m. a roof fall was found in the intake at 14 break along 1B belt. No one was injured or trapped. Both agencies were notified.

A roof fall occurred at spad #40+40 in #7, #7-8 and #8 entry's in the #2 North East Main intake. The area was previously bolted with 6 ft torque tension bolts and roof screen.

A roof fall was found at approximately 10:40am during the weekly examination in the Whittaker Return from 2-34 to 2-34 1/2. The fall was approximately 75 feet in length and 16 feet wide in the straight of the entry. No other part 50 criteria applies.

Old West 2 Intake Bk 16 #8, Mine foreman was inspecting discharge line and found that roof fell in the intersection on top of discharge line.

A roof fall occurred in the second north off the second main east at S.S. 7+20. Between entry 9 and entry 10. The fall was 60 feet long x 18 feet wide x 8 feet high.

An unintentional roof fall occurred in 7 North Mains 44 to 45 Wall #1 Entry. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer in the mine roof.

Hot coal was discovered on a highwall creating smoke. No flame observed. No miners were working in the area. The hot coal was covered with dirt eliminating the issue.

Brake transducer, manifold failed. Manufacturer called to diagnose, changed out while evac took place out of the #2 skip. All miners were accounted for on surface at 4:40pm. 4:45pm #3 was back up and running, test trips performed, lowering of men back into mine was resumed.

An unplanned roof fall occurred in left return on #10 belt which extended from Spad #1478 & 1479 above anchorage. No persons were injured as a result of this accident.

At approximately 2:40 p.m. the emergency hoist went down due to electrical issues. No persons were injured or trapped. The hoist was repaired and placed back in service.

A roof fall measuring 40'L X 18'W X 7'H was found in the No.3 left X-cut over to the No.2 intersection In-By survey station No. 17530. All approaches to the roof fall were immediately dangered off.

An unplanned roof fall was discovered at the Main North #6 Entry at 199 block in the intersection approaching the 9S #3 Seals. The fall is approximately 16'W x 80'L x 8-10'H. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

A roof fall measuring 40'L X 18'W X 4'H was found in the No.8 entry of the Coal Branch No.4 Panel Bleeder Return Panel in-by survey station No.17290. Approaches were dangered off.

An unintentional roof fall occurred in 7 North Mains 29 to 30 Wall #1 Entry. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer in the mine roof.

A roof fall above anchorage occurred in the J-1 #2 Entry at 48XC extending into the crosscuts over to the #1 entry and the #3 entry.

Test trips of #2 resulted in multiple trip out of skip. #2 brought to surface, damage to bonnet discovered. Falling debris struck top of #2 causing damage. Bonnet and rope repaired, test trips passed, shaft inspected. NO injuries.

Spontaneous combustion smoker was found in Area 1. Had to take a drill over and shoot dirt over the top of it. The area was buried at 16:30 pm 0721/2020

A roof fall occurred in the number 2 track entry on the number 3 belt at break number 8 at survey station #256. The fall was approximately 20 feet wide, 55 feet long and 6 feet high. No equipment was involved and no injury occurred.

Roof fall in the 6 Left Intake (#3 entry) at 108 wall. The roof fall measured 12' high x 20' long x 16' wide. The fall was found and mine management was notified at 8:38 am. Mine management called MSHA at 8:45 am.

A non-injury ignition/explosion occurred at an abandoned/sealed underground mine MSHA ID 44-07306. The explosion in the abandoned mine is believed to have occurred during a severe thunderstorm.

Roof fall was found when investigating a water line leak in the No. 3 entry. There were no witnesses to the fall. Roof fell to the bottom of the Rehder seam approx. 10 ft. where a localized wet area developed.

At approximately 9:44 p.m. the service hoist went down due to electrical issues. All persons were evacuated and no one was injured or trapped. The hoist was repaired and placed back in service.

An unplanned roof fall was discovered on ML 10 Belt at 78 block intersection. The fall was approximately 60' L x 30' W x 10' from the original roof line. No one was injured as a result of the fall.

Gravel quarry crusher cought fire due to hydraulic line breaking. Soaked running cat motor and ignited. Two workers saw this happen, one ran for extinguishers and other notified supervision which in turn called 911 and had local department on sight within 15 minutes. Crusher and one conveyor could not be saved. No personnel were injured.

At about 12:55 AM, the entire plant lost power. The transformers at the welding shop were sparking and had caught fire. The transformers were on the ground and the remnants of the cross members were smoldering with an occasional, small flame. Once power was pulled the fire was extinguished.

The #6 Portal elevator went down at 9:15 am with a malfunctioning Slow Down Switch. The switch was adjusted to the proper position and checked by Schindler Elevator and put back in service 10:10 AM.

An unplanned and reportable roof fall occurred at the #30 break, #3 entry on the F-11 Longwall panel (33 Butt). There were no injuries, no equipment damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

An unplanned roof fall was discovered in the 6 North Left Return at the 16 block intersection. The fall is approximately 25'L x 10'W x 8'H. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

Production hoist tripped to manual on a brake fault that was unable to be reset. Pinion switch was cleaned and lubricated. Production hoist was reset and tested.

A shipment of dry sawdust used for fuel was delivered to the site on 7-11-2020 at 4 PM. The load is delivered in a 53 foot semi-van container and is left in the container for distribution to the process at a different time. Later that evening at 11:00 PM a Minntac employee noticed smoke coming out of the van. Minntac employees put out the fire quickly and put people on watch.

Smoke was discovered in the 5 1/2 West coal pit at the top of the coal face. The smoke could not be extinguished in 30 minutes. No miners or mining equipment were at risk during this event.

During weekly examination of the hoist, examiner found the lead contactor was blown on the brake cylinoid.

A roof fall occurred one crosscut inby survey station 220 in the #3 entry on the #2 conveyor belt. The fall was approximately 60 feet long, 22 feet wide and up to 4 feet high. No injury occurred.

An employee was entrapped on the School House Portal elevator. An old coil shorted out when it rubbed against the frame in a bridge rectifier that leads to the brake motor. The employee was out of entrapment and off the elevator at 9:27 am.

An unplanned roof fall occurred in crosscut between survey Spad 1474 and 1477. No injuries or equipment involved.

Company #223, 993K Loader S/N Z9K00471 caught fire and could not be extinguished within 30 minutes. No injuries involved.

Employees doing maintenance cleaning ditches and culverts. The tumble weeds on were lit on fire, out of nowhere a wirly wind picked up one of the weeds and placed it on the old reject pile that had been reclaimed with straw. Some of the straw caught fire, a fire line was built using equipment and water trucks were used to extinguish the fire that lasted about 30 minutes.

An unplanned roof fall occurred between survey spad #1463 & 1464 in the track entry (secondary escapeway). There were no injuries as a result of this accident.

A fire was reported at around 7:45 am. The source of the fire was an old, abandoned underground mine fire. An ember from the old mine must have blown into the dry grass surrounding the hot area on the side of the hill. The fire was mostly contained and extinguished the next day. No one was injured and no mine equipment or structures were damaged by the fire.

During the pm/inspection of the service hoist, a crack in the yoke for the service hoist brake system was found.

EE was in elevator in Primary crusher and it stopped between floors E and D. EE notified control room via Gaitronics. It took some time to locate the key to move the elevator and open the door. EE was in the elevator for approximately 90 minutes. The door had been pryed open to pass in a water bottle while waiting for the key to move the elevator and open the door.

One employee was trapped on the Metz Elevator when the power knocked. The solenoid for the brakes got stuck and an elevator services company figured out the issue. The employee was off the elevator at 4:03 PM.

Roof fall occurred on the D-3 mains in the #3 entry, #3 crosscut, and measured approximately 20' in length. 20' in width, and 8' thick. This area is not an escapeway and is only traveled by a weekly examiner. No ventilation or travel was disrupted by the roof fall.

The BMX slope hoist was taken out of service when the ABB drive malfunctioned. The ABB drive was repaired and the hoist went back into service.

There was an unplanned roof fall at the 3rd West #1 entry spade# 2+80.

Roof fall above a anchorage point in the belt line of b-mains wall 89. There were two geological features and some water present in this area. The length of the roof fall was approximately 20 foot 6 inches by 65 ft. 21 ft. high above the normal roof.

Fire occurred to the High Angle Conveyor (HAC) when power was lost to the plant. A set of triple airlock gates and a knife gate separate the belt from the hot gases. The knife gate failed to shut in fail-safe mode when a valve was inadvertently left off. The HAC belt ignited and burned near a Continuous Emission Monitoring Station. Six compressed gas bottles heated and ruptured.

The explosion vents on the coal mill cyclone triggered and hot coal was pulled into the coal mill dust collectors. The kiln was shut down and the fire department called to wash the coal out of the dust collectors. The membranes on the explosion vents need replaced and the dust collector bags need replaced due to water damage when the coal was washed out of the hopper.

An unintentional roof fall occurred in a crosscut along 4 west belt line. The fall occurred at 45 xcut. The fall was approx. 40ft long by 20ft high by 16ft wide. This fall was caused by deteriorated geological conditions. There was no injury or equipment damage as a result of this incident.

29 MINE MAN HOIST BEGAN MAKING A NOISE ON 7/2/2020. MAINTENANCE ISSUE OF BULL GEAR KEY WELD HAD BROKEN LOOSE. MSHA: 10:15 AM - EMPLOYEES EVACUATED OUT OF THE MINE FOR REPAIRS. We were replacing the Cuttler Hammer electric brake that did not function as expected. The brake released and allowed the cage to drop pulling the salt skip onto the headframe crushing the bonnet.

Two related roof falls were found on 7/1/2020 in the bleeder entries of the CB#4 Panel. One roof fall occurred in the #6 entry inby spad 17394 and measured 20'Lx18'Wx4.5'H. The other roof fall occurred thru the crosscut directly inby between #6 entry & #7 entry (spad 17435) and measured 30'Lx18'Wx4.5'H.

A front-end loader operator was attempting to clear a buildup of sand from a stacker conveyor belt's tailpiece when the operator's arm became entangled. The victim was air lifted to a trauma center where the victim died a week later.

While loading a face on level 3, NW2, the man basket of the powder truck raised up into the roof of the mine causing a fatal incident. The accident is still under investigation by the operator and the Mine Safety and Health Administration.

Contact with electrical current

Employee was working in a de-energized 4160 volt GFCI controller in an attempt to reverse the polarity of the wires when it somehow became energized.

When trying to use cable winch on D-6 bulldozer, thought the operator saw them when they approached winch to unhook winch cable hook.

While servicing the Terex Vibratory Screen, the miner was positioning self from one side of the screen to the other when the screens eccentrics rolled causing EE to pinch finger.

The crew was changing belt out on 8 left, EE was installing keeper pins in bar of winder it was not aligned properly when it moved to align it caught EE's little finger at the finger nail.

Struck by powered moving object

Person was injured during routine cleaning of pin mixer. Machine was locked out, operator did not wait until it came to a complete stop before opening door. EE used EE's foot to get through buildup in front of the door and a pin attached to the spindle struck EE's foot. Broke some bones, caused a large laceration. One toe had to be surgically removed.

While driving the front end loader toward a stock pile of material the left front tire ran over a large rock jolting the operator in the cab.

The employee went today to a hospital because of a pain in left thigh and the doctor gave the employee rest until August 6. The hospital relates pain to work.

Employee was unloading bags of sand on the Battle Run Portal bottom. Employee was walking with a bag of sand on shoulder when they rolled their right ankle.

Employee working in a sand dryer repairing the refractory lining. Stepped on a sharp refractory anchor that penetrated the sole of EE's boot. Puncture wound rt. foot.

Employee went over the discharge line and twisted right knee.

Over-exertion in lifting objects

Employee was loading roof bolter. EE reached for and lifted a bundle of plates with one hand. EE felt a pop in right shoulder. EE continued working until 9/11/20 at which time EE began to miss work to have surgery.

Struck against a moving object

EMPLOYEE WAS OPERATING A 482 DIESEL SCOOP OUT-BY ON THE INTAKE TRAVELWAY AND FELT A PULL IN THEIR BACK.

Over-exertion (Not Elsewhere Classified)

EE was lying on the ground trying to loosen bolts on a D-10 dozer with an extension, when the ratchet slipped, caused employee to try and regain balance and hurt back.

Contact with electrical current

Employee was placing trolley pole onto the trolley wire when EE inadvertently contacted the energized trolley wire with EE's head causing electrical shock. It did not involve any compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Struck by falling object

Employee was installing the second bolt in the last row when a piece of undetected draw rock fell on left wrist and forearm resulting in a bruise. Employee was diagnosed with hand pain.

A fully trained miner was working on the miner when a rock fell and struck the employee in the back. The injury was not the result of any compliance issues nor were any protective guards involved.

Fall from machine

Truck driver was loading track pads that had been on loading pads while, three miner were at lunch. Driver was using a Serco 7000 clam loader and was seated in the operators seat when the bolts holding the boom of the machine snapped and driver was thrown from the machine landing on the pads below.

Employee was cleaning up work area at CV-09/10 transfer chute prior to taking a break. Employee placed EE's hand on an I-beam and the chute door that was leaning against another I-beam slipped and smashed employees right ring finger between the I-beam and the chute door. The result was a laceration and fracture to the right ring finger.

Employee was at the 1 West dust hole when EE was struck by a 4"" bull hose from the dust tank after filling the dust tank with dust causing a fracture to the lower back, a contusion to the right ankle, and a contusion to EE's left wrist.

Strong winds were blowing carbon flakes around. One hit the associate in the left eye when EE was laying drip lines with the pad crew.

Employee was turning a ball valve for the 1.5 inch water hose. The pressure was released quickly and the hose jumped and struck the employee in the right shoulder. This caused a minor sprain and the employee was given 3 days off and returned to work.

A fully trained miner was picking metal out of the loader when a rock fell and hit EE in the hand. The injury was not the result of any compliance issues nor were any protective guards involved.

Fall down stairs

The employee was walking on the main crusher's walkway, and slipped going down the stairs. As EE tried to stop the fall, EE landed on top of a sledge hammer handle in the lower level. The handle caused an open wound on EE's left thigh.

Cleaning around plant, dust particle got under contact lens. Employee was wearing appropriate PPE at the time.

Caught in, under or between a moving and a stationary object

Employee was lifting fork of extended reach lift to adjust width of forks. EE dropped fork into place. EE's hand position allowed the tip of pinky finger of EE's right hand to be crushed between back of fork and support cross member of backstop.

Employee was installing a roof bolt when EE claims EE felt a ""pop"" in EE's left shoulder due to overexertion. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Struck by falling object

Employee was helping to build braces for the crusher. The piece of angle iron employee was holding fell on employee's neck. Employee was taken to urgent care. They found no serious injury. Employee was given pain medicine and sent for an xray.

Employee was stacking stone in box off of the chopper & smashed a finger when loading the bucket

A crib block fell off the top of a bobcat and struck the employee on the foot and ankle.

Mon. 7/27/20 around 2pm EE cleaning & fixing things in hydraulic pump room & attempting to move a die press & couldn't handle the weight when it shifted & fell to ground knocking EE down & hitting rt foot. It swelled up immediately, EE went to cabin & iced & elevated. Took OTC pain reliever. EE declined flight out to have it checked. EE decided Tues 8/4 should get it checked.

Struck by falling object

The employee was checking the charge on a scoop in the 1WTG Construction Area, 60 block track heading. An employee on the other side of the batteries dropped their lid, the injured EE's lid fell pinning a hand between the lid and the batteries. The injured suffered no fractures and only a small laceration.

Over-exertion in pulling or pushing objects

Employee was closing garage door - straining arm.

Struck against stationary object

Employee was running scoop charger cable when employee stepped on a rock causing employee to lose balance, employee tried to regain footing and employee's right knee buckled causing a torn ACL and torn medial meniscus to right knee.

Fall to the walkway or working surface

EE was shoveling at the tail of a surface belt. When throwing a shovel full of material onto the belt, slipped backwards and hit elbow on the cement floor. XRay to elbow was negative. EE returned to work. Elbow has healed but still has pain in shoulder. EE had an MRI done on 8/21/2020 and has began missing work 8/26/2020 due to pain in shoulder.

EE was walking from 6 Mill Cyclone Deck to 7 Mill for EE's rounds when EE stepped with EE's left foot & felt sudden pain as EE's foot bent inwards. The grating was bad which made EE step wrong. EE was taken off work.

Employee's foot was stuck in mud / material, EE started to fall and let off the blaster hose that EE was spraying causing EE's momentum to go forward causing EE to fall.

Struck by falling object

Employee had just pushed pan and was going to pull up shields tight to the face. The coal and rock face fell into the already full pan. The rock hit the top of the spill tray and some came over onto the shield toes. A piece 2' X 1' X 1' hit miner on the side of left knee causing three fractures. Two below the knee and one above the knee.

Struck by flying object

Employee was helping other employees change out a conveyor when a rock that was lodged in a roller broke loose and shot out at the employee, hitting EE in the chin. EE received 6 stitches and 1 day lost time.

Over-exertion in lifting objects

Employee was shoveling when employee felt pain in left elbow & noticed swelling. Employee continued to work for several days & decided to seek medical attention on 7/27/2020 after the swelling & pain had not subsided. Employee became a lost time accident as of 7/28/2020. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Over-exertion in lifting objects

EE was shoveling dirt from the floor onto a non-moving conveyor in the surge tunnel and suffered a muscle strain in upper thigh

Struck against stationary object

Employee was unhooking pump hose and slipped on freshly graded rip-rap causing employee to roll ankle.

Employee was adding structure while the tail was chained off. The chain broke causing the tail to push the stucture into EE's ankle. EE was treated by EMT's and transported to the surface. EE went to the ER and found EE had a broken ankle.

The loader operator went to close the door on the loader, and smashed finger. This resulted in cutting the end of middle finger on right hand.

Over-exertion in pulling or pushing objects

Employee was pulling cable and felt pain in mid-section. Employee did not miss work until 8/7/2020 when employee got surgery.

An employee was installing a hose wrap on the hydraulic hoses up on the drill booms of a roof bolter. In the process employee's right pinky finger hyperextended and employee felt a pop. It was later discovered the employee has a torn ligament.

A fully trained miner tripped while on the beltline and injured bicep when miner fell. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was trying to break up some lodged anfo in the bed of a bulk anfo truck while employee was using a shovel employee had a sharp pain travel through shoulder and neck area. Employee started missing work on 8/12/20.

IE's finger was caught between a cross beam and a leg on a steel set while setting the cross beam. Miner was fully trained, wearing PPE and acting in compliance with the law.

Shift was ending soon and EE was returning to the bulk shack to collect their things. Noticed a camlock dust cap lying in the walkway. EE was handling camlock when they cut their middle finger. EE went to the lunchroom for 1st aid. Recommendation made for EE to be seen at medical facility for further evaluation. After evaluation it was deemed that seven stitches were required.

While tightening lug nuts on a mantrip, the wheel stud broke causing the employee to lose balance and to fall rolling the right ankle underneath EE. EE finished the shift and was taken to the hospital on 7/24/2020 where EE was referred to an orthopedic specialist on 7/28/2020. There EE was diagnosed with a distal fibula fracture and was taken off work.

Two surveyors were traveling up the slope out of the pit when the personnel carrier lost power and began drifting backwards. The ride impacted an embankment causing it to rollover and pin the passenger, causing fractured ribs, and head trauma. This resulted in a lost time injury.

IE was picking up a ""sailboat"" out of the pan line on LW setup when EE felt the muscles in EE's lower back tighten up. IE finished the shift and declined medical attention. IE woke up the next morning and decided EE needed to be checked out.

Miner operator suffered severe injuries to upper R leg.

Employee was using an air lance to poke through a thermocouple port opening on a plugged preheater tower cyclone, when the air was turned on to the air lance, hot material poured out of port opening splashed on employees Nomex hood around the right ear and face area. The heat from the hot material radiated through the Nomex hood and caused burns to both these areas.

Employee was turning in reverse on electric pallet jack and hit left fifth finger on metal tote behind EE. EE was taken to hospital and laceration was sutured. EE returned to work the following day with modified duties.

Employee immediately tried to move the man-lift to get away from the pick. The picking eye failed dropping the wear plate. Struck the employee in the side of the face causing two fractures that do not require surgery to fix.

Employee felt pain in their knee when they were walking down a slope and stepped onto a treaded liner on 7/24/2020. Employee felt reoccurring pain and on 1/27/2021 it was determined that the employee had a torn meniscus consistent with the 7/24/2020 incident that needed medical treatment.

Miner was operating a wheel loader to load the primary plant crusher when they ran over a large rock. This caused the loader to slam down with tremendous force, jarring the lower back of the miner and causing pain.

Employee was going down 3 steps from a trailer after a rain. EE's boots were muddy and EE slipped and fractured bones in left ankle.

A pipe line had been taken apart, flushed out, and bolted back together. Pump PU-03 was started up to flush the pipe line and water hammer occurred. It ruptured the poly pipe line, the pipe struck one mechanic on the back of EE's right lower leg. The injured employee was medically evaluated and placed on temporary restricted duties.

Employee was working to replace a pipe elbow. While positioning the coupling, employee experienced pain the shoulder while making adjustments to the coupling to be installed. It was not specified at the time of this report as to which shoulder.

The injured miner was jack hammering out a choke up in the throat that goes into the quencher dryer when received chemical burns from clinker dust. EE did seek immediate medical attention but the pain got worse later on and they went to an urgent care clinic the next day.

Employee was working on screen #4 repairing a drive motor when a piece of consolidated limestone dust fell down onto employee from conveyor #33 located above screen #4. Conveyor #33 was not running at the time of accident. The piece of consolidated limestone dust struck employee in the back.

When the two employees went to set the screen down onto the platform, one of the employees left hand got caught up on the left top corner of the screen slicing through the glove EE was wearing through EE's left middle finger and EE's left ring finger. The injury resulted in 9 stitches to the left hand.

EE was ball charging #12 mill. EE found the ball bin was plugged with seized 3 1/2 inch steel balls. EE attempted to dislodge the plug by throwing a freed steel ball up the chute. EE's hand came into contact w/the steel chute plate. EE fractured the pinky finger.

EE was hydro blasting the FC-7 drain line. EE was hit with high pressure water to right index finger, middle finger and palm while snaking out the drain line. EE was taken to a specialist and bandaged. EE was taken off work and received restrictions.

Employee was assisting with the removal of a screen side board from the basement of the plant when it slid down a post it was leaning against and caught the employee's right foot. The employee was wearing metatarsal boots at the time.

As the employee was operating the Wagner 25X, #9, the door on the operators compartment came open. The employee stopped the machine and reached out to the top of the door and slammed it shut. As the door shut it smashed EE's right hand between the door and the cab of the machine. Stitches were required.

Employee was in front of the steps at B tank manifold when EE tripped on an uneven surface. EE stuck EE's arm out to break EE's fall and jarred EE's arm.

EE was struck by 5 lb hammer head while removing coupler from pump shaft. Alternating striking coupler & other EE's hammer head came off the handle striking injured EE in pelvic region. Injured EE didn't seek medical attention on day of injury. Sought treatment on 7/27/20 & was referred to a specialist. Contusion to the pelvic region above the private area was the diagnosis.

Employee slipped and fell while holding the end of a 60-80 lb pump. EE's left hand was caught in between the pump and the concrete ground injuring EE's left middle finger.

A Mine Engineer sustained a fracture to right lower leg while operating the JLG Manlift. While tramming the manlift drove over a rock which caused them to unexpectedly get jostled in the basket, causing them to come down on right foot in an awkward manner resulting in the injury.

Employee's pinky was put back in place and EE returned to full duty the next day. On 7/27/2020 EE was re-examined and the pinky was not set correctly. Pinky was reset and employee was placed on restricted duty.

Employee was transferring oil from one gearbox to another using an oil transfer pump. When pulling the hose out from the inlet side of the pump, they felt a pop to right arm.

The Section Pressure Pump Breaker dropped out. EE attempted to put the power back in on the pressure pump and received an electrical shock in right hand. EE is currently working on restricted duty.

Employee was manually installing roof bolts. Was in a man basket inserting mechanical roof bolts, using an impact to install bolts, and using torque wrench to tighten roof bolts per spec. Shoulder became sore in the process of setting approximately 80 roof bolts in a shift and was still painful 2 weeks later so employee reported incident on 8/16/2020.

The injured employee was using a Dingo TX 525 to remove accumulated material beneath a structure. While operating the equipment in reverse, the bucket contacted a vertical steel member. The sudden motion crushed EE's left wrist between the Dingo controls and the diagonal structural steel member. The diagnosis was a strain and contusion of the left wrist; no broken bones.

The injured miner was removing broken bags from under the palletizer area when EE injured EE's lower back.

The employee was removing a silencer off of a dust collector fan with a coworker and as they were moving it out of the work area the employee took 2 steps backwards to the left and hit left leg on the adjacent pipe that was connected to the dust collector motor. The incident took place at 11:30 AM on Monday and was only reported on Tuesday morning to the employee's supervisor.

Haul truck operator explained that they had issues backing up and dumping (lever getting stuck) the HT585B when the bed came down unexpectedly at the Molinar Leach dump.

Employee was installing 10 inch pipe in the shaft, the pipe shifted away from them the employee tried to hold the pipe in place and felt a pain in left shoulder. Employee worked at full capacity until surgery on the 28th of September 2020.

EE lifting/maneuvering loader axle and felt pain in right shoulder up into neck area. EE placed on restricted duty on 7/13/2020 after medical treatment.

Employee was replacing a ripper tooth on a dozer and while striking the tooth to loosen it, a shard came off and entered EE's left leg just below the knee. It severed a vein and it required medical treatment to repair it.

EE was helping to move a MRS that was stuck in bad mine floor conditions. EE, along with several crew members, were assembling and spacing the metal slats and clevises to connect the C/M to the MRS. While handling and positioning the slats and clevises EE suffered a strained back. EE could not remember the exact time of the event.

Miner was replacing cutting edges on a 980G loader and in the process EE was pulling the center plate from underneath the bucket so it can be picked up with a boom. As the miner released the cutting edge and stood up, EE felt a ""pop"" in the lower right side of the back. This resulted in a muscle strain in EE's lower back.

While tightening a union on a pipe, the wrench came loose causing the EE to fall backwards causing a small fracture to their Fibula on their right leg.

The EE had one foot placed on the loose end and the other caught under the cordage causing EE to trip. When EE fell EE impacted the ground with EE's right shoulder.

Injured employee was helping another employee that was using pry bar on toggle plate when Injured employee hand was pinched between thumb & back of hand. On 8/10/2020 Injured employee reported that hand was hurting needed to go clinic get checked out. On 8/24/2020 Dr put splint on.

Was clearing chunks off of the top of the hopper with a shovel when product sloughed off and dust blew back into the employee's face and under safety glasses.

Employee forgot to step down when exiting a portable rest room, resulting in a fall. Employee strained right shoulder and initially returned to work without seeking medical evaluation. The next day, July 8th, the employee requested to be evaluated. Restricted duty was assigned.

Ee was being loaded by shovel. The first bucket lifted the bed and when it came down the ee was jostled in the cab. EE was seen at the ER and reported a shoulder injury and back injury. Diagnosed with shoulder and back strain and released to modified duty.

Injured was welding in the surface shop. A piece of steel fell and hit EE's right foot. An MRI was performed on 7-17-2020 and EE was placed on restricted duty at that time for a Non-Displaced Fracture in the foot.

While operating a manlift from one location to another, driver hit a dip that caused driver to bounce up with left foot landing on the material in the basket resulting in a sprain to calf from the twisting. We are reporting this incident because driver was prescribed physical therapy that driver attended on 7/30/20.

An operator of a CAT 777D Haul Truck was parked to be loaded by a CAT 992G Loader. The first bucket of material contained a large rock. When the bucket was loaded into the haul truck, the large rock caused the truck to shake. This caused the haul truck operator to experience neck pain.

Employee was loading a bulk truck. When trying to open a lid one of the hatches was bound up/stuck. The employee could not find a ""hatch tool"" and tried to manually force the latch open. Employee felt a pain in shoulder but finished the shift. Incident was reported the next morning when the pain had not gotten better.

EE was calibrating level indicator on condensate line. While taking off the back of the indicator, the nipple from tank broke spraying employee on both forearms and leg. Employee was treated and released back to work with restrictions to stay out of heat and keep clean.

Employee cutting bearing with grinder, cutting wheel. Wheel broke and piece flew off, cutting employee on right forearm in two places. Requiring stitches.

While assisting packaging coal, employee was placing 40# bags of coal into sealer and felt pain and soreness in left arm/shoulder area. On July 7, 2020 employee's arm was still sore and employee was taken to local Urgent Care and diagnosed with pain in left shoulder. Employee was given lifting restrictions and currently working on restricted duty.

Employee was turning a wrench to tighten a u bolt on spray bar, the wrench came off the bolt head causing the finger to get pinched between hand and screen

Employee lost footing after refilling the printer, causing a loss of balance and falling. The employee caught all their weight on their right arm which resulted in a fracture.

Employee was walking around a forklift and tripped and fell on the ground. Employee reported to a supervisor immediately but did not want medical attention. 6 days later employee's side was still hurting and employee saw a doctor who determined employee had fractured a rib. Employee is on restricted duty.

The employee was working on the vent dust collector and was going from hopper to hopper and was standing on metal grating. When EE turned to move EE's foot stayed where it was as EE continued to twist and EE heard a pop in EE's knee.

We had a cut piece of metal to make lifting lug, didn't get sharp edges ground down. Was going to take it loose from pump cover after it was removed and the cover wasn't secured and started falling, and the employee grabbed the metal and stuck sharp point in hand and it jerked EE's hand and cut it.

REPORTABLE ONLY DUE TO SUTURES - Employee was installing a roofbolt on 6 section, when EE pushed the bolt in with EE's hand, EE's knee hit the fast speed lever catching right index finger between the bolt and plate causing laceration requiring sutures.

EE was using a Hytorc wrench to tighten mill shell bolts. EE's left ring finger tip was caught between the mill shell and the wrench.

Completing a work order to replace the bags on L91-BF3. The cages and bags were sized to the cookie sheet and while trying to remove them with channel locks with jerking motion, with two other people helping, employee strained right shoulder.

Employee was raising mast on light plant when the cable broke inside the mast itself and employee jumped out of the way, banging left shin on the outrigger of the light plant causing a 1 inch laceration requiring one staple to close.

Employee was climbing a ladder while performing water line maintenance in the 65 722 shop. EE mis-stepped on the ladder causing EE to contact EE's chin on the ladder. EE was seen by the site medic, referred to ED for sutures to close the laceration and was issued a full release to return to work.

Injured Employee was working on a 32"" tall elevated platform without any handrail protection or fall protection. While hooking up a 1"" choker the tension of the choker pushed back and pushed the injured mployee off of the platform landing onto left side on the ground causing a fracture to the ribs.

Welding wear liners in drop chute of jaw crusher. Liner slipped & pinched finger between liner and other plate.

Employee was bench testing a radiator cooling fan. The fan became unstable and the employee went to catch the fan. The employees hand came in contact with the rotating blades and lacerated 4 fingers.

Employee was on their back removing grizzlies from the upper deck with an air impact with extension and socket. Employee had impact in left hand when the nut came loose and the impact wrench spun around hitting employee in the face under the left cheek.

Employee was running belt to check it and a lot of dust came from belt getting into EE's eyes causing redness and irritation, this belt had just been changed out is the reason for the dust in the raw coal side, EE sought treatment on 7-30-20, returned to work on 7-31-20

Employee was carrying a chain to the primary feeder to wrap around a rock. The employee tripped and fell on same level. EE caught the fall using right wrist causing EE's wrist to fracture.

Employee was changing engine oil in a triaxle. EE was working in the lube pit when the accident occurred. A drop of oil fell between the employee's safety glasses and eye. Hence, employee got a drop of oil in EE's eye.

The worker was striking the head of a bolt with a hammer to unplug a 1 ¼ X 4"" pipe nipple. When the material inside the pipe nipple broke loose the impact of the bolt pinched the thumb of the left hand against the pipe nipple. The worker was transported to the local emergency room and received 5 stitches to the thumb.

Employee was stopping the 1st 60 North belt when a tailpiece jack fell and struck right foot. This resulted in a hairline fracture of the 4th toe on the right foot.

Was helping attach Huesker mesh to the shield tips, in preparation for LW recovery. As EE stepped across the panline they felt a pinch in lower back. The following day EE was stiff and sore. EE was examined at Medical and received Chirpractic treatment from their own chiropractor.

MINER WAS TRAVELING TO SURFACE WHEN EE'S LEFT EAR CAME IN CONTACT WITH THE MINE ROOF, RESULTING IN A LACERATION, WHICH DID REQUIRE STITCHES TO EE'S EAR.

The employee's left little finger was mashed between a water line coupling and the roof bolter bumper. EE was treated at Broaddus Hospital where EE received two sutures to the little finger. The employee was released to work full duty and returned to work on EE's next scheduled shift 7/25/20.

Driller and helper removed rotation guard to straighten it. While removing it the top half of the guard folded down and pinched Driller's right thumb. Driller had cut-5 gloves on. X-Rays showed no damage to right thumb. The doctor cleaned and wrapped the wound and prescribed an antibiotic. Employee returned to work with no restrictions.

EE was welding in the HSI crusher. After welding for approximately 4 hours, EE states that a rock ""the size of a softball"" became dislodged from the crusher above EE and struck EE on head.

member was lifting bucket into the jig when strained a muscle in EE's back after taking it easy for 15 min EE went to valley immediate care to have it looked at. the doctor said it was a strained muscle and to take it easy until the 08/01/2020

Employee approached compressor to unload and shutdown. As they approached, the connection between compressor and hose failed (safety pins, and whip check were in place) and the hose impacted left upper arm, resulting in a puncture wound from a bolt on the coupling clamp. Employee received 6 sutures to close wound. Returned to work on next shift.

Employee was exiting the crew bus at the ready line prior to start of shift. EE lost balance and hurt right ankle when stepped out on to the ground.

Employee was adjusting fly ash system when a piece of material blew into EE's eye. EE was checked by a physician and received a prescription antibiotic ointment for precautionary purposes. This injury report was overlooked during furloughs, etc. amidst the COVID crisis.

Miner was installing a rubber guard. The rubber was too large. Miner is right hand dominant. Miner held the rubber in place with right hand and used a utility knife in left hand to cut the rubber. The knife slipped and cut the middle section of ring finger on right hand. Miner received 3 stitches.

The employee dropped something while using a torch. EE bent over to pick up the item and the flame from the torch burnt EE's arm. EE states should have turned the torch off before retrieving item. Employee has a 2nd degree burn on right arm. No lost time from work.

The employee was performing a bucket calibration test on the kiln 1 and 2 lime silos. A trash bin had been filled with lime and was being moved to a scale to be weighed, when the trash bin was set down - lime puffed up from the bin and got behind EE's safety glasses and into EE's left eye. Employee was seen at ER and prescribed antibiotic eye drop.

Employee working the mix pot. When employee turned around, employee felt that something went into right eye. Employee informed the supervisor and we flushed employee eye with solution, then we flushed the eye under the eyewash station.

Installing a pony belt-drive at a new location. EE was choked up on a sledge hammer tightening a large wedge on top of cribs in the center of the drive, the rib was 30"" away. EE swung the sledge striking the wedge, the handle bounced back striking EE's left little finger against the frame of the drive, causing a laceration.

As the forklift operator retracted the boom, the fork tips contacted the bottom of the locker causing it to tip and fall backwards landing on the employees back. Laborer felt irritation in L eye while walking in the plant. Didn't know the source of the irritation & didn't try to treat eye until arrived home after shift. When they woke up next morning eye was still irritated & swollen so they decided to report condition when they arrived at work. 07/23/20, we sent them to doctor which prescribed antibiotics for eye.

Employee was driving back to shop on main haul road. Approaching Dixon ramp 2 they got too close to the edge of ditch and got pulled in causing truck to overturn on its side. The employee was wearing a seat belt. EE received a small laceration to the side of head requiring 3 staples.

While troubleshooting a set of scoop batteries, the battery cover lid fell catching right hand between the battery frame and the lid. Taken to hospital for exam where they were referred to a hand specialist on 7/22/2020. Employee was treated for a laceration to the right hand requiring sutures, and was released to work full duty.

Employee was splitting rock when a piece fell on the down button while hand was pulling the stone away pinching hand between the plate and the stone. Employee did not report incident until 3:15pm end of shift. Taken to hospital on 7.22 am for exam and drug screen was placed on modified duty same day.

Employee was walking at the west spoon fuel island and tripped on uneven ground, stiff armed the ground with left hand. At the time of the report EE stated EE was fine and that EE just wanted to report it.

EE was walking to pump barge, slipped/fell with right knee striking a rock.

A large piece of metal had lodged in the cone crusher North while trying to remove the metal, the crusher turned and smashed the employee's thumb against the bowl.

IE was rotating a hoist drum coupler by hand to align the halves and pinched thumb between them causing an avulsion and tuft fracture that required stitches.

Absorption of radiations, caustics, toxic and noxious substances

Employee was accessing the Reclaimer building. EE felt something bump hard hat, then back, then back leg. When EE looked around to see, there was a bat attached to EE's leg biting EE's pants. EE stated never felt a bite or sting. Due to the high mortality rate of rabid bat bites (6-8% bats are rabid) EE decided to have the vaccine administered.

Employee was operating the 40 RTD performing their last clean up for day shift around the 40 shovel, when they were headed up the ramp to park for shift change the front end of the RTD fell into a ditch. This caused the employee to pitch forward hitting their head on the ROPS causing a laceration and breaking the right side window.

Employee was attempting to adjust seat in a PC490-11 excavator with right hand when weight shifted forward pinching right little finger in the frame of the seat. Resultant was a cut to the tip of right little finger. The employee was treated with surgical adhesive and a tetanus shot. Employee returned to work that morning with no restrictions.

An employee mashed the end of the right middle finger when EE caught the finger between a piece of top conveyor belt structure and the conveyor belt structure rail. This resulted in breaking the tip of EE's right middle finger.

Employee was using water hose to wash tail pulley and during this task it splashed back at them and got in an eye causing a scratch. Employee was wearing safety glasses.

Cutting steel 1""X6""X10', steel started to fall, moved hand out of way, caught finger on saw blade resulting in five stitches, saw was not on.

Employee was removing a differential and mounting a grease line. The line came off of the mount and fell on the employee's finger.

Foreman was feeling the inside of a hole that had been reamed out using right middle finger. Foreman was making sure there was no residue in the hole. As they had the finger in the hole, the porta-power that was supporting the push pole slipped and allowed the push pole to drop. The employee suffered a distal tip amputation sparing the finger nail, there was no bony exposure.

Equipment operator was using a pry bar to clear jam at crusher when bar slipped and they smashed finger requiring 14 stitches.

Injured employee was advancing shields on the 2W L/W face while mining toward the tailgate end. Piece of rock fell from the gap between the shields and struck employee's left thumb while hand was on the hand railing. Employee failed to remove hand from the railing at the shield gaps as they were walking along the face. Thumb was fractured at end and cut requiring 3 sutures.

Employee was injured while installing a sprocket on a drive shaft in the maintenance shop. EE was striking the sprocket with a hammer when a piece of metal approximately 1/4"" in size sheared from the sprocket and stuck into EE's right arm. EE received one stitch to close the wound. EE was wearing gloves, safety glasses and a hard hat.

Team member was hammering a tooth into an adapter with a single jack style hammer. The hammer impacting the tooth caused a metal chip to fly up resulting in team member retaining a metal fragment in chin. Injury was originally first aid. Medical treatment was not provided until 8/11/2020.

Ee was pushing down the dig face on the 5380. The dozer hit a rock in the face, causing it to stop abruptly, and the ee's teeth to bite down/clash together, fracturing a molar on the lower left side. Seen at the dentist who confirmed no unusual decay causing fracture, determined work related.

Employee reached down to help free the miner water hose where it was tight pulling around a corner. The hose slid quickly and a coupling mashed employee's finger tips. Employee ended up getting one suture in finger.

While employee was reeling in a water hose at the back of a water truck. A metal containment box lid fell and smashed the employee's left index finger resulting in stitches.

Employee was repairing the Downender on the #1 stripping machine at Central EW tank house, when the cable broke and the arm came down hitting the employee's left ear.

On 7/14/20 employee was putting EE's hardhat on when the cap lamp battery came unhooked from the back of the hard hat swinging around and contacting EE's right upper incisor tooth chipping it. Employee continued to work EE's shifts, while on days off EE had the tooth repaired on 7/28/20 making it a medical reportable incident at this time

Employee was crawling under a D11 dozer to help replace a hydraulic line. At the end of shift employee reported discomfort in the knee. Doctor recommended an MRI. MRI results today revealed a torn MCL. Normal work without restrictions or time off at this time.

An employee was using an auto retracting knife with hook blade to cut through a liner weld to inspection it As EE cut the liner the blade met resistance at the weld seam. EE pulled harder to cut the weld seam. The blade cut through the weld seam and struck EE in the left leg. EE got a 2"" cut which required 8 stitches and a tetanus shot.

Flash burns (welding)

IE was treated on 7/13/20 for flash burn of the eyes, but did not notify mine personnel until after being treated. After receiving doctors notes on 8/3/20 it was determined this was a reportable incident as medications were administered to their eyes.

Miner was attempting to remove a rock that was in the collection box at the top of conveyor #3 at the primary before plant start-up. Employee was prying a rock with a bar that was about 6"" that was wedged against other material. While the miner was pulling the bar, the rock became free and the bar pinched EE's pinky finger on the left hand between the top of the box and the bar.

Putting scoop on charge raising battery lids and lid fell back down hitting EE's right hand and thumb, causing a closed fracture to very tip of the thumb.

While an employee was splicing a 42"" conveyor belt using a belt cutter, the cutter became unbound and slipped due to the excessive force. The employee's forearm contacted a double headed nail that caused a laceration.

The employee was in the process of removing a screen bearing housing from a shaft.  While attempting to get a chain mule hooked up, the housing slipped off the shaft.  The bearing housing was secured and hanging by a chain fall. The bearing housing slipped off and swung to the left and caught the employees left hand between the bearing housing and the timing gear shaft.

Employee was torching a part on the cone crusher when a piece of debris got into EE's eye. Employee was following company policy and wearing safety glasses. Employee reported the injury the following day 7/11/2020 and was sent for medical review on 7/13/2020 to evaluate.

Service Technician was performing maintenance on an impact crusher, rotating the impact blow bars. While handling one of the bars EE lost grip and it slipped out of EE's hand. A sharp edge on the blow bar cut the palm of EE's hand, requiring 5 stitches to close the wound. It was a very hot day and EE decided not to wear protective gloves.

IE was using pieces of concrete block to fill in the holes on the sides of the wall IE was building. As IE was using a rock hammer to fit a piece of block into place, IE struck left index finger between the block and the hammer, causing a laceration to the index finger. IE was taken to the medical facility for treatment where IE received sutures to close the wound.

The employee was assisting another employee with changing out some screens on the crushing plant. One of the screens had a sharp edge where it was worn down from the rock and had broken off. The sharp edge of the screen punctured the employee's right hand.

Employee was installing driveline on Caterpillar 769 Haul truck, driveline fell on outside lower right leg above ankle.

Performing engine repair on 992C Wheel Loader. Cleaning air filter with sanding pad attachment. Piece of metal flew into Left eye. Eye became irritated next day. Given Rx and referred to ophthalmologist 7/13.

Ee was helping a mechanic with a bed pin on a haul truck. EE was pushing the pin in, and it gave way, causing EE's finger to slip between the bed pin and the bed of the truck. Seen at ER, x-rays negative for fracture, and stitches required to close the cut on the finger. Released to full duty.

EE was attempting to lock a clamp when the wrench being used to grasp the clamp handle slipped off the handle and sprung back striking the ee in the forehead. The ee suffered a suturable laceration to the forehead. Returned back to work full duty.

Struck against a moving object

HT loaded by shovel, 1st bucket the door opened unexpectedly, material fell on the dovetail causing the bed to raise up, slam back down. HT driver experienced leg pain upon impact, was seen at the ER for general body aches, given a work as tolerated release. On 11/30/20 employee informed the mine operator that EE had been receiving physical therapy which started on 10/09/20.

Delivery driver from Rhinehart Oil preparing to pump oil from truck tank to Dicaperl tank. Truck bed has raised seam which cause delivery driver to trip and hit head on a rail.

Worker was pushing a wheelbarrow on a semi muddy surface lost footing and dropped to right knee, knee made contact with a stone approximately the size of a lemon, which caused a laceration on the skin below the knee cap.

Fall to the walkway or working surface

Great Lakes Trader employee was standing near mooring wire preparing to move wire off of mooring bit. When a second Trader employee started the winch, the mooring wire struck injured in the stomach, knocking them to the ground. When they fell to the ground, hard hat fell of and they struck head on the ground.

While replacing a lift cylinder on a haul truck the associate's right middle finger was pinched against the frame causing a small fracture in the tip of the finger.

Struck by flying object

While operating the tailgate drum on the shear at shield #30 on the 8th South Longwall, The employee was struck in the back of the head by a lump of coal that had flown over the raise sloughing plate on the shear. Employee received a laceration which required medical treatment (Staples).

Employee was using the impact in right hand and got cut on the left middle finger. Employee received stitches.

Struck by flying object

While mining in the 8th South longwall panel #10, there was a bounce and outburst of coal at the tailgate at shield #136, which resulted in the mining equipment being down for more than 1 hour. One person was injured and received medial treatment (stitches).

Employee was attempting to roll rock and adjacent rock dislodged and pinched right 4th finger.

Struck by falling object

An emp. was using a scaling bar to remove rock cores left in the back by the continuous miner. The employee was unable to knock down the core and walked underneath it, the core fell and struck the the emp on the hard hat. Emp. taken to ER for a concussion and given prescription meds in the ER and prescription to take home.

Struck by flying object

A piece of metal chipped off while working on a track and cut employee's leg approx. 2-3cm in diameter that required 4 stitches. Employee returned to work with no restrictions.

Caught in, under or between a moving and a stationary object

EE was replacing blocks and shims under a plant support. EE's thumb was under a shim when the jack was lowered. EE was taken to the hospital to have 4 stitches in right thumb.
