Accident type, without injuries

Due to an electrical problem associated with the oil heaters, the oil to the hoist motors got too cold, preventing the hoist from operating properly. The hoist was inoperable for a period of approximately 60 minutes before the oil was back to normal operating temperature.

HOISTING INCIDENT : Cowling Portal Elevator - On Thursday, 11/30/06 at 12:40 pm the cage was taken down & was inoperable for more than 30 minutes. The elevator was not properly hoisting. The door switch on bottom was worked on to repair the problem. MSHA was notified at 01:20 pm on 11/30/06. The elevator was operable & running on Thursday, 11/30/2006 at 02:00 pm.

Fall was approx 40'L X 20' W X 4'-6' thick. Torque tension bolts were the primary support. Fall was approx 15,000' from nearest face. No injury or blockage of ventilation. Fall will not be cleaned up. Cribs and timbers will be used to prohibit travel through area.

A bounce occurred around the longwall headgate area of the Gil-5 panel. The bounce separated top coal from the roof rock, measuring approximately 20' x 40' x 24-30"" thick. Production was interupped for more than one hour while the area was re-bolted.

Hoist down- a brake pressure transducer on the service hoist had a loose wire causing a voltage drop. Evacuation was called underground. The problem was fixed in approximately 40 minutes, before any employees evacuated the mine. MSHA field office in Newark notified at 1:39 p.m.

A roof fall occurred on the 3 Left 4 West panel at crosscut 39. The fall was located in the crosscut between the roadway and 5A belt. The cause of the fall was weak laminated shale. The fall was located adjacent to the roadway and there was concern about the intersection so the roadway was re-routed around the fall area.

Contacted at home that the power was out again at the mine. All underground personnel at primary escapeway and accounted for. Contacted MSHA Boise office and left message. Avista Utilities restored power at 8:15pm. Normal operations resumed. MSHA Boise office called and updated with a message.

A roof fall occurred on the 3A travelway between crosscuts 51 and 52. The cause of the fall was weak laminated shale. The fall was located in the pillar line between the crosscuts and not the intersection. The travelway was re-routed around the fall area.

A fall of roof occurred on #1 unit in the 4th East panel at x-cut 41 in the last open x-cut. The fall measured 50' X 19.5' X 8'. The fall did not affect personel, ventilation or equipment.

A methane ignition occurred on the active working section MMU 001, in the face of the No. 1 heading off the #2 Northwest Mains. The ignition occurred while coal was being extracted with the Joy 14CM-10A continuous mining machine. No fire needed to be extinquished. Nobody was injured.

A coal outburst occurred approximately 70 to 75 feet outby the longwall face on the beltline. The result of the outburst moved the tail piece and end of the stageloader, obstructing the walkway from the longwall face and this caused a disruption in mining for more than an hour. There were no injuries caused by this outburst.

A fall of roof occurred in the 1A Panel in the # 5 Entry, between x-cut 31 and 32. The fall was approx 35ft long x 19ft wide x 8ft high. The distance from the fall to the face was approx 980ft. A hidden slip with water was determined to be the cause of the fall.

The 1085 level hoist became temporarily inoperable due to a brake sensor out of adjustment. Normal wear (vibration) on the brake sensor caused it to go out of adjustment. The sensor was readjusted and the hoist was operational. There was no damage to the hoist.

On 11/28/2006 at 3:00 am we incurred a roof fall at 1 + 80 cross cut of #6 entry of the 3rd SW panel off the 3rd MNW in unit #2 (MMU 002-0). The roof fall measured 40'L x 19'W x 12' thick. The fall involved unit scoop #3 being covered from the battery end to the cab of the scoop. The scoop was unattended at the time of the fall.

A roof fall was discovered at spad location 1727 off the 2nd south main in the 1st left panel. The area is approximately 85'long x 20'wide x 6' high. The fall had no affect on employees, ventilation nor equipment. The area has been dangered and cribbed off.

Contacted Boise field office supervisor that the power was off at the mine. All mining activities were halted and all underground personnel were proceeding to the primary escapeway to wait. Avista Utlities restored the power to the mine and we were able to resume regular operations at 10:00PM.

An unintentional roof fall occurred on the 10th West panel at crosscut 52+60 entry #2. The fall did not affect ventilation, passage of men or cause any personal injury. The fall measured 36 feet long x 20 feet wide x 8 feet thick. The area had been roofbolted with 6 foot fully grouted roof bolts.

A roof fall was discovered in the fourth crosscut outby spad 4737 in the #1 entry leading to the cabin hollow bleeder. The fall is approximately 70' long x 20' wide x 6' high. The fall had no affect on employees, ventilation, nor equipment. The area has been dangered and cribbed off.

On 11/27/2006 we incurred a roof fall at 56+30 x-cut of #5 entry on the 3rd M.N.W. The fall measured 40' x 19.5' x 7'. The area was developed around 11/6/2006. The fall was called in 11/27/2006.

Contacted at home that power had gone out. Only 2 men underground. Both EE's at primary escapeway and accounted for. Left message at MSHA Boise office. Power restrored at 1:00am. Power out again at 2:10 am. No EE's underground. Decision made to cancel dayshift. Power restored at 7:30am. MSHA Boise office called and they were given an update.

A fall of roof occurred measuring 0-10' high X 12' wide trapping the mobile roof support. The ventilation was not disturbed and the fall will not be cleaned up. The fall occurred on the pillar section 001 MMU #5 pillar block at SS #8425.

5 Mains 39 block, #4 entry 4 way - MMU 003. 18 ft. wide x 20 ft. long x 8 ft. high. Support: 6 ft 3/4 inch resin bolts, pans, straps, screen, 12 ft cable bolts. Fall dangered off, additional support installed around fall.

An unintentional roof fall occurred in the #2 roadway entry in the G6 section at x-cut 20. The fall had approx. measurements of 35'long X 25'wide and 20'above the original mining height.

An unplanned roof fall occurred in the #4 entry of the 2 Left Rooms 2 x-cuts inby survey station #420 and passed through x-cut to the #3 entry. Roof fall was approximately 55' long, 18.5' wide, and 4' plus high. Fall did change our course of travel in the return. Area was dangered and timbered off.

Cutting machine was cutting #1 entry when water started coming in. We evacuated the men & made provisions to pump water. After the water had cleared, it was determined we had cut into a cone hole drilled from above.

On 11/21/06, an unintentional roof fall occurred on the 3rd South Sub Main travel way, #6 entry between #60 and #61 cross cut, which is the secondary escape way for #1 unit. This area was supported by 10' and 15' point anchor roof bolts, the roof fall measured: 20' in length, 18' in width and 12' to 15' in thickness. This roof fall will be cleaned up and the area re-supported.

Examiner found roof fall in #2 entry of 1st E/main north at +1945. Fall is in primary escapeway - intake - and will not be cleaned. Escapeway set over to #1 entry. Approaches to fall have been timbered. Fall is approx 6 feet 7 inches high x 18 feet 8 inches wide x 40 feet long.

Raw coal stockpile showed signs of heat. A front end loader was brought in to open up and investigate the situation. It was determined that there were several areas that were burning. After further evaluation, it was decided to contact the local fire department for assistance. Hot areas were seperated from the stockpile with mobile equipment and cooled down with water.

The main distribution cross arm broke causing a power failure at the mine. No one was entrapped in the shaft or shaft pockets. Power was restored at approximately 9:55 PM.

An unintentional roof fall approx 20' long X 16' wide X 7' high. Occurred in the #3 entry at the base of the highwall.

A fall was discovered in the No. 6 return entry of No. 1 North just inby survey station 1205. The fall measured approximately 60' long by 19' wide by 6' high. There were no injuries or equipment involved; ventilation and passage was not affected. The mine is idle and the fall will not be cleaned up.

Examiner found roof fall in #2 entry of the main north entry fall is approx. 40' long 17-18"" wide 6-7' high fall is in intake between 32 to 33 xcut 2580 to 2660-will clean up around fall in adjacent entries to have passable travel wall. Fall will not be cleaned up timber or cribs will be set on ends of fall.

Section C-9. On 11/20/06 an unintentional roof fall occurred in #7 entry, #16xc, intersection of the 3rd South Sub Main return entry. This area was supported with 10' point anchor roof bolts. The roof fall measured 23' in length X 18' in width X 14' thick. The roof fall will be cleaned up.

A roof fall was discovered on the 3 north main at the #21 crosscut in the #4 entry. The fall measured 20' by 20' and was approximately 12' in thickness.

Unintentional roof fall 2nd Main East Intake in the X-Cut #4 to #3 Entry 1 row in #3 intersection 154 X-Cut. 18' long x 20' wide x 6' high. 4 Way intersection. Proped between travelway and fall. Entry not normally traveled.

Hoistman noticed water in tunnel that houses electrical cable for hoist in subfloor of #1 hoist room. Did not want to energize hoist with standing water around cables; electrical and shaft maintenance group pumped out water, hoist energized. No miners were allowed to go underground during this time, and no miners were underground when the potential problem was discovered.

A reportable roof fall was found in the F-Seam, entry 5, between crosscuts 59 & 60. The fall was approximately 20 feet wide, 6 feet high and 60 feet long.

A roof fall occurred on the 3A supply road, No. 6 entry, at crosscut 15. The fall was later determined to be a part of a differential settling event. The area was sealed.

Emp. was changing batteries on R/C #74 at the charging station between 302 & 303 S. While lifting new battery, sparking occured and a fire ignited the wiring on the R/C. Emp tried to extinguish fire but couldn't. Emp then notified authorities. Down 8:15 p.m. - Back UG 11:44 a.m.

A Methane Bleeder was ignited in the mine floor at the No.6 Shield on the walkway side of the panline while the shearer was cutting coal at the headgate. No injuries or property damage was incurred.

An unplanned roof fall occurred on the #1A belt. There is no active face at this mine. No injuries were reported. No equipment was involved. Passage of persons and ventilation were not affected. All areas of fall have been breakered off.

The material door on the man cage was not completely closed, so when the shift started underground the door hit the beam at the collar, bending the door inward making the cage unsafe to use until the door was repaired, in about 2 hours.

The fall measured 60' long by 20' wide by 6' high. There were no injuries or equipment involved. Ventilation was not affected. The fall will not be cleaned up.

An EE was riding in the elevator at the preparation plant, when the elevator stopped between the 5th and 6th floor. The EE did not exit the elevator until a broken coil wire was repaired and the elevator was placed back into operation. There was constant verbal communication with the EE and no injuries were reported.

On 11/14/06, an unintentional roof fall occurred in #8 entry, #24xc. Intersection of the 4th North Sub Main return. This area was supported with 10' point anchor roof bolts. The roof fall measured 22' in lenght X 18' in width X 10'-11' in thickness. The roof fall will be cleaned up.

86 Mack cabover carrying solaire air compressor for drill, air compressor caught fire, took nearly 30 min to put out, fire department was involved, no injuries. Two employee's from Givelich Explosive were just starting to drill shot when air compressor caught fire.

During an onshift examination, the section foreman discovered an intersection with deteriorating roof conditions. The intersection was dangered off and the condition reported. While the intersection was dangered off, and before any additional support could be delivered to the area, the roof fell in.

On 11/13/2006 a roof fall was discovered at Station 54+90 intersection of the #0 entry, of the 3rd M.N.W. The fall measured 50' x 19.5' x 7', the area was developed on 11/6/2006. The fall was called in on 11/14/2006.

Unintentional roof fall discovered at 5:50 AM. Fall measured 6' to 10'H X 19'W X 65'L. Originally supported with 6' full grout resin bolts in entry and 6' full grout and 10' cable bolts in intersection. #5 track between 36 & 37 block, Main Hdgs, to right of stations 4441 - 4448. Reported to C. Mosley at 5:55AM 11/13/06.

A roof fall occurred on the Franks Creek Mains. The fall was at break #72 and #6 entry, 1 break outby spad no. 13593. The fall was 4.5' thick, 8' wide & 25' long. The fall was located in a neutral airway. Fall was cribbed off and extra cribs were installed in surrounding area.

A roof fall occurred measuring 20' X 75' X 7-8' thick at SS 1526 intersection thru right crosscut in the #4 entry. No equipment was involved, stopping repaired, no injury occurred and the fall will not be cleaned up at this time.

On 11/13/2006 at 8:30 AM a roof fall was discovered at 54+90 x-cut of #5 entry, of the 3rd M.N.W. The fall measured 12' x 18' x 6'. The area of the fall was developed on 11/6/2006. Called in on 11/14/2006.

Section C-9--On 11/13/06 an unintentional roof fall occurred in the last open cross cut, #7 entry, #33xc to #34xc. This area was supported with 6' and 10' point anchor roof bolts. The roof fall measured 50' in length X 18' in width and 12' to 14' in thickness. The roof fall will not be cleaned up. All approaches to this fall have been cribbed off.

Control power to production hoist tripped, resulting in skip being closer to the top than indicated(normal when skip trips @full speed).Hoistman raised loaded skip at excessive speed,skip ran through overtravel due to inertia.

On 11/13/2006 a roof fall was discovered at station 56+30 x-cut of #0 entry of the 3rd M.N.W. The fall was called in 11/14/2006. This area was developed on 11/9/2006. The fall measured 18' 18' x 8'.

A roof fall occurred in the crosscut #48 of 7th left section. The fall was above the anchorage point. The fall was estimated at 17' wide, 20' long & 20' high. The fall is a result of loose laminated shales. This was the case above the anchorage zone also. The fall was cribbed off & will not be cleaned up.

Roof fall is at spad # 1487. Fall occurred in intersection in #4 entry in the first northwest mains. Fall is approx 20 x 50 x 7' high.

A new roof fall occurred on the South Mains ""B"" beltline between 28 and 29 x-cut. The fall was approx. 8 to 9' high by 18' wide and 60' long. Area was bolted with 8' Install III bolts and resin, H-beamed with 4"" beams sitting on cribs. Area was cleaned up and re-supported. Fall did not obstruct ventilation or escapeways. Cause of this fall was weather deterioration.

Employee was using a Joy continuous miner to cut & grade the bottom in the #1 entry on #6 section in the intake, approx. 1000' outby the face when an ignition occurred in the bottom on the left side of the entry. No injuries occurred.

Found roof fall on the 1A road at 8121' to 8041' of the north entries. Roof fall is 90' to 95' long, 17-18' wide, 7 to 8' high. Also it fell across 1A belt roadway. Had additional support previously installed (timber and cribs). Primary roof support was 6' tension rebar. Fall will not be cleaned up.

A fall occurred on the 8O section in the intersection of the #4 monkey butt on the 8O set up face. The fall was 20 wide by 25 to 30 feet long.

On 11/9/06 an unintentional roof fall occurred on the 4th Main West, 182 cross cut number five entry. This roof fall was supported with 10' point anchor roof bolts. The fall measured 20' long x 18' wide x 10'-15' high. The roof fall will be cleaned up and resupported.

At approx 3:15 am on 11/9/06 a roof fall occurred at station 7 + 70 crosscut on the 2 mwm at #1 entry.

A roof fall occurred in the 5 East Panel at cross-cut #17 in 1/2 the intersection of #3 entry extending 10ft. into the pillarline toward #2 entry. The fall measured approximately 20ft. x 20ft. x 5ft. The area was supported with 5 ft. fully grouted roof bolts and 9 ft. mega bolts. An undetected slip was determined to be the cause of the fall.

Maintenance activities were proformed on the hoist during earlier shift. The emergency stop buttons were not reset following completion of the maintenance work. Therefore, the hoistman could not start the hoist. The emergency buttons were reset and the hoist operational. There was no damage to the hoist.

A roof fall measuring 20' L x 30' W x 9'H occurred in the in the #4 entry in the 10th Rt. Panel at Spad # 5490 on 7C belt. Area was bolted with 5' TT Bolts.

Apparently, a small confined methane face ignition occurred on the 003-0 section in #3 entry right crosscut of the 9 Right Panel off of 5 East. No injuries occurred. After the investigation of the incident by MSHA and company personnel, no signs of ignition, burning or charring were observed.

The 1085 level hoist left brake #2 cartridge valve had become stuck. The cause was a low break pressure alarm that did not immedialely reset. The alarm was eventually reset and the valve has since been replaced. The hoist was in service within 25 minutes; however, MSHA was contacted within 20 minutes of the hoist being down. There was no damage to the hoist.

Frictional ignition occurred on the No.1 LW Face at No. 90 Shield. The shearer was cutting bottom with the headgate drum, when the ignition occurred under the H/G Drum. The 1085 level hoist was inoperable for approx. 4 hours due to corrosion on the auxilary switch on the scroll latch, which caused it to fail. Because of the faulty switch, the digital hoist supervisor on the PLC dropped the ""WD1"" relay & would not reset properly. Maintenance replaced the switch & reset the PLC. No injury or equipment damage.

The electrician was in the process of trouble shooting an electrical problem in the line of power feeding the 8200 Dragline. At the time of the accident the victim was in the KM-12 junction box when he contacted live electrical components resulting in fatal injuries.

He was pulling a shuttle car cable that had been wrapped around the canopy post on the left side of the operating deck. As he was pulling the cable, the rear wheel of the scoop ran over the cable. This caused the cable to unwrap & strike him on the side of his head.

Employee was installing a strap when his canopy hit the top causing a rock to fall, rock struck his index and middle fingers on his right hand. Index finger had to be amputated.

Employee informed company management on Monday, Nov 20th that he would like to see a doctor. On Nov 21, 2006 the company doctor diagnosed the injury as an inguinal hernia.

Employee was in the process of helping another ram car out of underpass. The underpass was very steep. The batteries on the ram cars became interlocked. Employee was exiting the cab of the drill. He stepped down on the 1st step, then missed the 2nd step & fell about 4', landing on the ground. He stated that he was unable to grab the handle in time to keep him from falling. Other than failure to maintain 3-point contact, there appear to be no contributing factors causing the incident (moisture, wind, defective equipment, etc.).

EE was helping load a mining component onto a dolly. He was standing on the right side just outby the dolly that was to be loaded. When the other workers started to move the mining component onto the dolly, the outby end moved forward & went off track. The dolly sat down on the employee's feet. He was diagnosed with a fracture to the metatarsal (middle) in each foot.

Bodily reaction, (Not Elsewhere Classified)

Employee was on his knees and in process of installing a roof bolt. When he started to get up his right knee popped and locked into one position and could not bend.

Caught in, under or between a moving and a stationary object

Employee was loading material in loader bucket and caught hand between material and bucket.

Struck by falling object

EE was repairing the jaw crusher. He was straightening out one of the grizzly bars with a sledge hammer. A rock fell through from above him somewhere. It hit him above his left ear and knocked his hard hat off.

Struck against stationary object

Member was in a hurry crossing the line of the longwall when he ran into one of the longwall shields in front of him that he did not see due to his head being down. He struck his right shoulder against the shield causing pain in his back.

Fall from piled material

Employee was walking up mountain to go get a hose. The mountain was very muddy due to the weather. He slipped and fell in mud on knee and rolled about 10 foot down hill.
