A fall of ground occurred in UE 159 after blasting the heading. The ground fall fell approximately 15 ft back from the standing face and went up in the back approx 10-12 feet high.

A roof fall occured in the 15 north panel, #4 entry in the intersection of crosscut #16. The fall measured approximately 20'x19'x5'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occured in the 15 north panel, #4 entry, starting in the intersection of crosscut #22 and ending in the intersection of crosscut #25. The fall measured approximately 185'x18'x5'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the accident.
AN UNINTENTIONAL ROOF WAS DISCOVERED BY THE MINE EXAMINER IN THE 2ND WEST SECTION, INTERSECTION OF THE #2 ENTRY AND X-CUT #12. THE FALL WAS APPROXIMATELY 18' WIDE x 18' LONG x 6' HIGH.
An unintentional fall of roof was discovered during a weekly exam on Big Block Submains break #5 in #2 entry. The fall was app. 7' x 20' x 20'.
The Kuhntown elevator stopped 90-100' below the top landing. Breaker was re-set and ran in inspection mode to get 16 employees on elevator off. Contact nips were replaced and elevator tested several times and cleared for normal operations by MSHA.
An unintentional roof fall occurred in the 8I Tailgate. The fall occurred @ s.s 117 + 40. The fall measured approx 80 ft x 16 ft wide x 4' high. *No injury or equipment damage*
The roof adjacent to the last shield on the face fell in and blocked travel into the tailgate. The face was located at survey station 12+16 at the time of the fall. The fall went approximately 5 ft outby the face in the TG entry.
While pillar recovery mining, a roof fall occurred that covered up the remote control continuous miner.

A roof fall occurred at #197 intersection in the #2 entry 1 block outby spad#1674.Foreman observed intersection was working. He dangered the area off, moved life line to adjacent outby, built crib as breakers & Cribbed adjacent interesection The area was bolted with 5/8 x 72"" full grouted rebar using 6 X 6 plates & 2 X 6 X 14 boards. The fall was 50' long x 17' wide x 6' high
A roof fall was discovered in the #6 entry of the 2-left panel off #2 main west (inby survey station 907). The fall measured 90' long X 20' wide X 5' high. There were no injuries or equipment involved. The fall will not be cleaned up. This mine is idle and equipment is being removed from the mine.
A roof fall was discovered in No. 2 return entry 300' outby the face area of the No. 1 South. The fall measured 8' H x 20' W x 85' L. Ventilation and travel was not affected.
Roof fall occurred in the South Submain, Entry #9, Crosscut #1. Fall was approximately 20' long, 20' wide & 8' tall. The area was bolted with 6' & 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment. This area is not activeworkings. No one is required to work or travel in this entry.

A fall of roof occurred in the 1st North West Mains at X-cut # 162, in the # 5 Entry. The Fall was approx 40' long x 18' wide x 10' high.
A roof fall occurred adjacent to the 4B belt on the 1 Left 4 West 2 right panel at crosscut 9 between entries 3 and 4. The cause of the fall was weak laminated shale. The fall blocked the travel route for examiners on the return side of the 4B belt.

A roof fall was found at crosscut 147 entry #2, 5 RMS. Fall was in four way intersection and was approx 30'l, 80'w, 10' h. Area had been mined 4/15/05 and was supported by 6 & 8' full resin bolts. No injury or property damage resulted.
Roof fall occurred in the 6th subpanel south off of east 1st panel south (old #4). The fall began at spad #5 + 20 and was approximately 120' long x 18' wide x 6' high. Date of development 7/06. Type of roof support 6' fully grouted bolts. Cause of roof fall was due to the roof deteriorating above anchorage zone of bolts.
1st left section #7 Entry 1-Break inby spad 10543. Rock caught CM in face cut. Area started working, crew retreated. A roof fall occurred and 3 rows of roof bolts were lost. The area was dangered off, fall reported & additional roof supported added.The CM was recovered with a hyd. retreiver. The fall was approximately 16'W X 35'L X 5'H. The bolted portion contained 4'resin bolts.
Two employees were riding the hoist into the mine when the hoist malfunctioned. The malfunction was due to a 24 V power supply that failed. The two employees were on the hoist approx. 60 min before it was repaired.
The pre-shift examiner found a roof fall at approx. 7:50/am in the 2nd Right section intake just inby #13 x-cut. The fall measured 16' wide, 20' long and approx 7' high.
Unintentional roof fall located on the Northwest Mains #2 Section, #6 entry, Station 5211. Fall measured approximately 18'L X 18'W X undetermined height. No injuries or equipment involved. Area previously supported with 6' fully grouted resin bolts and cable bolts. Overburden approximately 290'.
An unplanned roof fall occurred in the #130 Mine measuring 80'l x 30' w x 25' h.
A roof fall occurred in the 1 Main East in #1 entry in the intersection of cross-cut #141 measuring approximately 25ft. x 18ft. x 6ft. The area was supported with 6ft. tension rebar roof bolts. An undetected slip was determined to be the cause of the accident.
At approximately 5:00 PM the elevator at the Galatia North Portal went down when a wire came loose on the solenoid switch for the automatic brake assembly. No injuries resulted from this incident.
Friction along a pellet load out conveyor resulted in a fire.

An unintentional fall of roof was discovered during a weekly check on the 3rd panel of Right Fork submains, break #35 in the #4 entry. The fall was app. 35' x 7' x 20'.
Nothwest mains, entry 5, spad no. 269, Distance from active face: 2100', No personal injuries or equipment involved, Airway - neutral; size: L - 50', W - 19', H - 5'; 48""resin; Strata: shale, laminated, wet. All entrances are being breakered off. Area will not be rehabilitated.
A roof fall measuring 5 feet by 18 feet by 240 feet occurred in the 001-0 section return air course. It did not interupt ventilation.
On the afternoon of 12/21/06 at approximately 1:20 pm a rock burst in the 4000-226E stope occurred. By 1:35pm all personnel underground were accounted for & OK. Msha was notified at1:35 pm PST. The crew in 226 stope was withdrawn. On 12/22 the stope was examined. Minimal damage along approx 100' of footwall from face to raise. Area was scaled & slushed & bolts installed.
A roof fall measuring 50' X 16' X 5'-6' fell approx 1500' from face between entries 2 & 3 at spad #7519 & 7509. It was timbered and dangered off. There were no injuries.

There were no injuries or equipment involved and ventilation was not affected. This mine is idle and the equipment is being removed from the mine.
An unintentional roof fall occurred approximately one crosscut inby SS# 1294 between the #6 and #7 entries. Fall dimensions were approximately 40'L x 20'W x 5' high.
A non-injury roof fall occurred at crosscut 14 of lowest between the #2 and #3 entry. The fall was over 8' high, 16' wide and 40-45' long. Additional support had been installed and water was observed coming out of mine roof.
A roof fall was discovered in #9 entry 1 break outby spad 373. The estimated size of the roof fall measured 6' high, 18' wide and 70' long. No injuries where reported as a result of this roof fall.
An outage of the slope hoist occurred due to an electrical supply interruption. The elecetrical power and hoist outage was from approx. 4:40 am to 5:48 am.
A roof fall occured at spad 0+00 inby to 30' outby spad 1+40. The fall is located on #1 unit-1F panel in the #6 entry (belt entry). The fall is 110' long X 20' wide X 40""+ high. This was in the 2nd room set-up. Unit moved to the 3rd room set-up.
A roof fall occured in the 2 main north, crosscut #2, between #2 and #3 entry. The fall measured approximately 30'x19'x6'. The area was supported with 6' tension rebar bolts and 6""x6"" timbers. An undetected slip was determined to be the cause of the fall.
An unplaned, non injury roof fall was discovered on 12-20-06 at approx 2pm while the mine examiner was conducting his weekly examination. The fall was in the intake air course at the 57th cross cut in the #6 entry. The fall measured approx. 0-8 inches thick, 19 feet wide and 30 feet long.
A fall was discovered in the No. 1 heading of main west set of entries just inby survey station No. 30. The fall measured approx 200' long x 20' wide x 6' high and will not be cleaned up. There were no injuries or equipment involved and ventilation was not affected. This mine is idle and the equipment is being removed from the mine.
Federal Mine Inspector was examining worked out area (2 H Rooms) when he found a cave 1 cross-cut inby spad 1339. Cave extended from R1 across belt to L1. Cave was 50' long by 18' to 50' wide by 5' to 6' high. Area supported by 4' resin bolts on 4' x 4' pattern, cribs, & steel posts. Ventilation not affected. Cave will not be cleaned.
A roof fall occurred on the 3 Left, 4 West panel in entry #6 at crosscut 37 to 38. The fall was located adjacent to the primary escapeway for MMU-005. The fall was in an entry where a set-over wall had been erected. The area was cribbed and timbered.
A roof fall occurred in #5 room, in 1-F panel; of #1 unit at spad #3+50 to 4+20. Approximately 80' long x 20' wide x 4' high.
Roof fall occurred in intersection between 24 and 25 seals in 8 B A Panel. Fall is a continuation of an old fall. Fall is approx. 16 ft wide and 9 to 10 ft high, extending to old fall about 25 ft away. (Found on weekly examination call out and reported to MSHA at 9:10 AM.)
A fall of the roof occurred in the 4th s.e. subpanel at cross cut #16 in the #2 entry on #1 unit. This fall was 40'l x 19.5'w x 10' high. No ventilation or personnel were affected.
An unintentional roof fall occurred at the New Future Portal on the 7th North longwall tailgate/7th North headgate, Entry #1. The fall was unknown length, width and thickness. The fall did not affect ventilation. The fall will not be cleaned up.
Lost power to #2 Hoist. When power to the hoist was restored, the level 1 sensor stop switch failed to function. The mine evacuation was complete at 9:18pm. The sensor switch was replace, the hoist was tested and miners ret'd underground at 12:55am
1085 hoist main AC breaker would not reset due to PLC output error. System was rebooted and all checks were completed. There was no damage to the hoist.
A roof fall occurred at Crosscut 81, Entry 6, 1st North Intersection. The fall measured approximately 20' long x 20' wide x greater than 9' thick. The fall did not result in any damage to equipment or property.
A roof fall was discovered by the fire boss at 4 Right 2 East at station 5228. The fall was approximately 6' high x 20' wide x 20' long. The fall will not be cleaned up. The fall will be breakered off.

Rock fall occurred at face of 001 Submains bleeder in the #2 entry. Fall measured approx. 8' wide x 25' long x 2' thick.
Roof material fell on the continuous miner while mining a push out. The material that fell was not caused by a coal or rock outburst. The 7000-1 is submitted for the sole purpose of abating citation 7259198 issued on 4/18/07. You should not construe our submission of the 7000-1 as an admission of a violation of 30 CFR 50.10. Company intends to challenge MSHA's findings in citation.
Hoist lost hydraulic due to pressure valve not working due to contaminates in oil. Valve was cleaned and system scheduled to be flushed out and oil changed.
After a pre-shift exam was made and prior to the face prep crew reaching the section at 12:10 AM, the crosscut 9 to 10 fell in. The fall was 20' wide x 40' long and 7' to 8' high. The area was supported with 6' fully grouted resin bolts and 2-12' cable bolts every 8'.
A roof fall occurred on the 2 Left, 5 West panel at crosscut #34 in entry #5. The fall was located in the roadway leading to MMU-002. The roadway was temporarily routed around the fall area and the fall was later cleaned up and supported and the roadway was returned to its normal location.

Enerfab ID# G941 was replacing inpact rollers in product silo using torch to remove existing rollers. Torch was last used at 4:15pm with a walk through inspection conducted at 4:40pm another inspectionat 4:55pm. Hot Work permit was in effect. During shift change eereported a belt fire. Due to dense smoke the fire department was called. Call was made to Arlington office.

The rock fall occurred in the #6 entry in the in the 5 North Belt in the #19 intersection. One entry to the right of spad #15747. The area was bolted with 5 foot resin bolts. The fall was 20 ft by 20 ft, 4 to 5 ft high. The fall was cleaned up an a canoply put in it.
The fall was in the return at 55217. Out by #2 drive in the return.
An unintentional roof fall occurred at the intersection of the #3 seal on the Main East seal line on the main portal bottom. The fall was approx 40' long, approx 40' wide and approx 7' thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up.
The 1085 hoist became inoperable due to a faulty relief valve. Previously a new relief valve was installed and it did not seal itself correctly. Hydraulic oil leaked around the valve and caused the system to overheat. The valve was replaced and the system was operational.
A short occurred causing the two 24 volt power supplies to trip out. When the power supplies tripped this faulted the power to the analog control loops to several field devices. The coal mill weigh belt stopped and the two tempering air dampers stopped. An explosion occured in the #3 coal mill or ductwork causing the explosion vent to rupture as designed.
Roof fall 100' inby mine mouth starting in #1 entry and continuing across same line to #4 entry. Fall is 130' long, 18' wide, and 12-20' thick. No injuries or damage.
ROOF FALL (7000-1-021-06) : LOCATION - Incline Switch, Slope Bottom Between Road and Belt. DIMENSIONS - H:5', W:19', L:30'. GEOLOGY - Soft, medium to dark gray shale. ROOF SUPPORT - 6' Conventional bolts. The area was mined in April 1973. Cutters and draw slate were present in the roof.

Unintentional roof fall occurred on the North Mains Section, MMU 001-0, in Xct to the right of Station 731, #5 Entry, 86 Block. Fall measured approx. 8'H X 30'L X 18' W. Area mined on 12/12/06. Overburden approx. 220'. Area previously supported with 5' fully grouted resin bolts. No injuries or property damage. Called fall in at 10:30 AM to MSHA and WVMHS&T
A roof fall occured in the 2 main north, #7 entry, in the intersection of crosscut #1. The fall measured approximately 25'x19'x6'-8'. The area was supported with 6' tension rebar bolts and cribs. An undetected slip was determined to be the cause of the fall.
The #8 elevator was inoperable for a period exceeding 30 minutes due to the door switch linkage out of adjustment. Adjustments were made to the door switch linkage and the elevator was placed back into operation.
Brakes on hoist drum would not release (main pump was changed along with 2 relief valves). All checks were made after change-outs and hoist is now fully operational.
A roof fall occurred on the 2 Left 4 West panel inby the section tailpiece between crosscuts 36 and 37. The cause of the fall was weak laminated shale and slips. The area was recognized prior to the fall and the area had been timbered off in advance of the fall.
A roof fall occurred in the 2n #6 Mains at XC #94 from #6 to #7. Fall fell across both intersections. The fall measured approximately 18' Wide by 10-12' Thick by 80' Long. The area was supported with 6' fully grouted bolts.
ROOF FALL (7000-1-020-06) : LOCATION - MW#1 Roadway, between Crosscut #9 and #10. DIMENSIONS - H:6', W:19', L:80'. GEOLOGY - Soft, medium to dark gray shale. ROOF SUPPORT - 6' Conventional bolts. The area was mined in March 1983. Cutters and draw slate were present in the roof.
An unintentional roof fall occurred at Crosscut 22 on the Galatia North Main West travelway at the 2250' mark in the #6 Entry. The fall was approximately 40 feet long, approximately 18 feet wide and approximately 8 feet thick. The fall did not affect ventilation. The fall will be cleaned up.

An unintentional roof fall occurred at Crosscut 121 on the Galatia North Main West travelway at the 13,651' mark in the #7 Entry. The fall was approximately 60 feet long, approximately 40 feet wide and approximately 8 feet thick. The fall did not affect ventilation. The fall will be cleaned up.
12/11/06 discovered fall 5 breaks outby spad #3723, in return. area will be rehabilitated, 12 ft. rope bolts will be installed at 8 ft intervals 200 ft inby and 200 ft outby. both omsl and msha were notified.
An unintentional roof fall was discovered on 12-10-06 in 002 intersection, #5 Entry and Spad no. 10860. The fall measured 20' in length, 20' width and 01 - 6' height.

An unintentional roof fall occurred at Break #61/Spad #1622. The fall measured approx 7' high X 30' wide X 40' long. It was discovered by the 3rd shift mine foreman, who was conducting a pre-shift examination.
A roof fall has occurred int he first southeast maws, in no.1 entry inby spad 10 & 50. The dimensions of the roof fall area 50' long, 18' wide and 19' high.
A roof fall occurred in the 15 North Panel in #8 entry in the pillarline between cross-cut #15 and cross-cut #16 measuring approximately 30ft. x 19ft. x 5ft. The area was supported with 5ft. fully grouted roof bolts. An undeteced slip was determined to be the cause of the accident.
Fire occurred in the area of #1 engine on RH340 excavator; the machine is a total loss. No injuries occurred from the fire.
ROOF FALL (7000-1-019-06) : LOCATION - Main D, Face Area, #5 Entry, 80' North of Spad #57429. DIMENSIONS - H:6.5'-7', W:18', L:18'. GEOLOGY - Soft, medium to dark gray shale. ROOF SUPPORT - 5' Fully grouted bolts. The area was mined in May 2001. Cutter and draw slate were present in the roof.
Control relay went bad; changed relay and it worked normally.
While mining 2 left, top started working and the miner was pulled back to safe area. A fall occured measuring 50' X 20' X 8'. No injuries occurred. Timbered off and moved section to another area.
We were mining 630' under the mountain there was a rock fall ont he head when we tried to drill it out the latches brase on beam lowering 18 beams and the mines heard 63rth undergound.
The production hoist developed a noise and was shut down for checking the niose out. The SCR pack was faulty. Production hoist back into service 12/9/2006.
On Thursday 12/7/06 at approx 10:20 pm a supply trip was being returned to the surface fromt he slope bottom. When the trip left the bottom, supply cars loaded with gravel wrecked which caused damage to the break car. MSHA and state agencies notified timely.
Elevator stalled between floors.
At approx. 7:30am the elevator at the Galatia North Portal had a limit switch actuator break off on top of the elevator. The elevator was out of service for approx. 3 hours until the part was replaced. Workers were transported to the main portal & went below there. No injuries resulted from this incident.
An unintentional roof fall occured in the #2 entry of the Main South Entries at the +2730 foot mark. The fall occured due to the fact that the roof strata broke just above the anchor point. The fall measured 20'x30'x6'. An investigation will be conducted in all adjoining areas for adverse conditions. The fall area will be cleaned up and resupported.
A roof fall occurred on the 2 section #3 entry outby the tailpiece 1 block inby spad 2884. The fall was 20 x 30 x 5. The top was laminated shale and sandstone with multiple cracks.
A roof fall occurred in the 2 North #6 mains, crosscut 93, #3 entry. The fall measures approximately 20 feet long, 18 feet wide and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts.
ROOF FALL (7000-1-018-06) : LOCATION - Main E Return, Crosscut #103. DIMENSIONS - H:6', W:19', L:30'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' Fully grouted bolts. The area was mined in June 2004. Prominent roof jointing in the roof were the apparent cause of the fall.
HOISTING INCIDENT : Cowling Portal Elevator - On Wednesday, 12/06/06 at 09:53 am, the cage was taken down and was inoperable for more than 30 minutes. The elevator was removed from service. The bearings on the idler roller were changed. MSHA was notified on 12/06/06 at 09:55 am. The elevator was operable and running on Wedneday, 12/06/06 at 02:00 pm.
A non injury roof fall occurred in the #30 x-cut of 10 west between the #1 and #2 entry. The fall measured 18' wide x 20' long and 6 to 6 1/2' high.
A non injury roof fall in the #3 entry on 9 west is blocking travel and safe access off the 10 west LWTG. The rock fell around the bolts and cribs. 4' to 6' high x 16' wide x 15' long. 32 to 33 x-cut.
A roof fall occurred at approximately 11:10 pm on Tuesday, December 5, 2006 outby the longwall face. The roof fall fell to the main roof and is approximately 20 feet wide by 45 feet long. The fall is located between the longwall face and spad 10082 in the No.6 entry. The area was supported with 6-foot resin bolts. (MSHA field supervisor was notified at 11:22 pm on 12/5/06.)
Discovered a roof fall at station 6+40 of entry #2 of the 1st N.W. Panel off the 3rd N.E. Main-Unit #1. The fall measured 120'long X 18'8""wide X 6'-8'thick.
The #2 hoist motor relay contactor malfunctioned causing the hoist to not function properly. The mine was evacuated at 3:30am and complete at 3:42am. The defective contact relay was replaced, the hoist tested and miners returned underground at 6:10am.
The balance chain that attaches chain to cage and counterweight failed (broken chain link). The mine evacuation was completed at 2:45 p.m. A new balance chain was installed and the hoist was tested. Miners returned underground at 6:35 pm.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE PRESHIFT EXAMINER IN XC 35, BETWEEN #3 & #2 ENTRIES IN THE 2ND WEST MAINS. THE FALL WAS APPROX. 35'L x 15'W x 10' HIGH, DOME SHAPED. AN INTAKE STOPPING WAS KNOCKED OUT BY THE ROOF FALL.
A roof fall occurred on the 2 Left 4 West panel in entry #4 at crosscut 30 to 31. The fall covered the 3A belt and structure. Due to the nature of the roof in this area and the history of falls in this area MMU-003 was removed from this panel and reclamation will commence and the area will be sealed.
Silicate Plant Tray Dryer (Zone #3) had unburned fuel that ignited, creating a small explosion. This created enough pressure to open most of the dryer doors. Discovered that the air flow switch had been jumpered out, allowing the gas to remain on with not enough air to burn all the fuel. No injuries were sustained and one section of the roof on the west side buckled (minor damage).
An inundation of water from old works, thought to be over 300 ft from the face occurred in the No.1 entry of the 001 MMU. Miner was backing out of a clean up when he brushed the rib and the water burst through.
A floor heave and coal outburst occurred at approximately 70 to 75 feet outby the longwall face centered at the tailpiece. Area involved 30 to 40 feet of rib and about 60 feet of floor heave. The result of this incident caused movement of the tail piece and stage loader reducing the walkway to 1 foot and disrupted mining for approximately 1 hour, no individuals were injuried.
A fall of roof has occurred in the 3C Panel in the # 4 Entry at x-cut # 3. The fall was approx 40' long x 30' wide x 8' high.
10-Right section #2 Heading at approximately 2:25 pm an ignition occurred in the face. No injuries, flames lasted about 5 minutes and 3 fire extinguishers were used to put out the flame. Line curtain was 8 feet from deepest point of mining and flames did not come back past the end of line curtain.
An unintentional roof fall occurred in #6 entry, #60xc, intersection of the 3rd South Sub Main travelway entry. This area was supported with 10' point anchor roof bolts. The roof fall measured 25'in length by 18'in width & 10'in thickness. The roof fall will be cleaned up.
A fall of roof occurred in the 1st North West Mains at x-cut 19 in the # 3 Entry. The fall was approx 18' long x 18' wide x 8' high.
A 20 foot x 20 foot x 7 foot high roof fall was discovered during the preshift in a crosscut #4 to #5 330 feet outby the E-7 working face. The accesses to the crosscut had been cribbed off and the crosscut abandoned. A pressure break was evident in the crosscut and extended into the #5 entry
After the failure there were slickensided or polished bedding contact evident in the immediate roof. The fall was approximately 5.5' high, 23' wide and the length included 20' of unsupported roof and extended out into 16' of permanently supported roof. Six foot permanent roof support was evident and hanging from the immediate roof after the roof failure occurred.
The 1085 hoist experienced electrical and computer problems. The PLC communications error made the hoist inoperable. There was no damage to the hoist.
A roof fall occurred app. 4000 ft outby active face between spad #2225 and 2229 in the #5 entry. Fall was approx. 90ft l x 20 ft w x 15 ft h.
The No 4 block in the last open crosscut of the number 3 entry fall is approximately 30 to 40 feet long and about 30 feet wide and about 12 feet high.
A fall of the roof occurred on #1 unit in the 4th N.E. Panel at x-cut 42 in #3 entry in the intake. The fall measured 40 ft x 19 ft x 6 ft. Ventilation, equipment or personnel were not affected.
A roof fall was discovered in #4 entry outby spad 31283. The estimated size of the roof fall measured 4 1/2' to 5' high, 18' wide, and 20' long. No injuries were reported as a result of this roof fall.
A non-injury foof fall has occurred in the intersection measuring about 18 foot wide and 22 foot in length from 2 to 7 foot in height.
On 12-01-2006 at approximately 7:45 PM on #1 section between no. 4 and 5 entries a roof fall occurred. The fall measured approximately 80' long, 19' wide and 8' high. Both MSHA and State officials were notified and the fall was investigated. All entrances to the fall have been timbered off.
At approx 12:10 am on 12/1/2006, a fall was discovered in 0 entry at station 52 + 10 of the 3rd main northwest. No injuries or property damage and no impairment to ventilation.
An unplanned fan outage occurred @ the Bailey Mine which exceeded 15 minutes. No injury or equipment damage.

A roof fall occured int he return of old workings. No equipment was in the area of the fall. The adjacent areas were timbered off to prevent the fall from spreading.
While traveling underground toward section, employees noticed top breaking and dripping and could not get stopped in time to avoid roof fall. Equip. operator stopped ride & started backing up when fall occurred, striking employee. #2 Main Intake 18 Bl., inby spad #34.
Frozen dig face material sloughed from the dig face of 750-28 bench, striking employee and pining him to the ground.
Lowering shuttle car canopy when canopy slipped it fell hitting rt hand (middle finger) between canopy and frame of car.
Employee was checking drive belt tension on jaw screen 13-367 (Plant was shut down). Employee pulled down on drive belt and pinched right middle and ring finger. Plant was locked out. **Lost tips of middle and ring fingers**
Partial amputation third metacarpal right hand. Hand caught in Pulley while replacing belt. Bearing Replacement on Conveyor Belt System.
The employee was operating the continuous miner by remote when some draw rock fell on the miner. The employee backed the miner out of the place in order to remove the drawrock. As he was pulling on a large piece of drawrock, a piece 37"" long x 14"" wide x 0"" - 3.5"" thick broke out and slid down onto his little finger and outside of his right hand.
Employee was dumping rock from his 773B rock truck (#63W1910) when truck overturned landing upside down, causing damage to the rock truck and sending him (via ambulance) to the hospital with a back injury.
A team was setting up the crane. An employee then put his fingers on the alignment collar-bolting flange to see if the bolt had broken off in the housing. While physically checking the bolt pattern the hydraulic cylinders were actuated and the alignment collar slid back into the cylinder housing smashing the left hand index finger of the employee who was checking the bolt.
Employee was pulling the belt pin (spindle) on a splice using a claw hammer when he was struck in the eye with a sliver of metal which required eye surgery.
The truck employee was operating was parked under the shovel, a boulder fell out of the shovel into the bed of the truck. Second incident - shovel dropped boulder on tail end of truck causing the bed of the truck to lift up and bounce back down. Employee's neck was jarred both times.
Employee sprained ankle stepping off of a tractor. Employee was diagnosed with sprained tendon and fitted with a cast during a follow up appointment on 1/4/2007. Employee's first missed work day was 1/7/2007.
While employee was standing on a shield, a co-worker pulled up another shield next to the one employee was on and while pulling the shield up, the tow bar in the shield hung on something, causing the tow bar to swing around and strike employee.
Employee was stepping down off a ladder. He slipped on a muddy step and sprained his right ankle.
Injured employee was washing around the plant using a water hose. He said that the pressure increased and he lost his grip, and the hose struck him in the left knee resulting in a contusion.
Employee stepped and slipped in a mud puddle after checking a chute. He felt right knee pain. Discomfort continued and an ACL tear was diagnosed. He missed his first shift on 2/23/07 after right knee surgery on 2/22/07.
Employee was working changing a trolley pole that was dogged at both ends. The pole (which was cracked) broke the rest of the way and raised up striking the employee in the chin. He received 5 sutures in his chin.
Employee was removing filter underneath truck when wrench slipped causing shoulder pain
While walking back to passenger van, employee twisted left ankle. This incident did not become recordable until 01/30/07 due review from medical.
Fell approx. 15-20' to the ground from cab of overturned haul truck. EE comments to EMT's indicated he experienced a physical episode affecting his consciousness or orientation prior to event. Incident under investigation.6 broken ribs, punctured lungs, 2 compression fractures of L1/L4, strain to right arm, lacerations to face/head, and damaged tailbone.
Injured EE and another EE picked up the cutting edge of the 80ZV front end loader (part weight is about 125lbs) and put it on the loader. Injured EE said oops, there goes my back.
Another ee was going to tram 3-wheels when he went in wrong direction pinning EE against bolter.
Employee complained of low back pain when he bent over to work on a junction box on the boon of a shovel. He was evaluated by a physician on 12/31/06 and was taken off of work.
Employee alleges that while operating a loader she received a neck injury. Employee claimed that she hit a railroad track with the lip of the loader's bucket stopping her abruptly and tossing her foward in the cab. Employee sought her own medical treatment and was taken off of work.
EE was originally injured on 12/29/04 at MSHA ID 4406991. This report was filed. EE now said he was operating his loader and when he changed direction, the loader jerked, causing some pain in his neck.
Employee was performing maintenance activities on a motor grader and while stepping off the machine twisted his right knee. Upon further medical evaluation the employee's injury required surgery on 2/23/2007 to repair a torn meniscus.
A piece of frayed wire from a hoist cable became embedded in EE's hand. He didn't start missing work until 1/9/2007.
EE punctured the palm of his right hand while working on a water line to the stool in the employee bathroom. He washed it out and bandaged it. Later he developed an infection. On March 2nd the infection was removed. This is when the injury was reported.
Employee reported on 1/6/2007 that he thought he had frostbite on his feet from walking in cold water over the course of a week.
Stepped on wooden pallet, board broke, causing injured to twist knee.
Employee stepped off a concrete foundation onto the snowy area and slipped, straining the back.
EMPLOYEE STEPPED OVER A PIPE LINE, FELL, TWISTING ANKLE. THIS WAS NOT AN REPORTABLE INCIDENT FOR THE ANKLE. THE EMPLOYEE LATER DEVELOPED A HERNIA PROBLEM FROM THE FALL THAT REQUIRED SURGERY ON 01/11/2007.
Operator was mounting rear of front end loader when he lost his grip and fell about 2 1/2 feet onto his lunch bucket breaking his left collar bone and 3 ribs.
Driver lost control of vehicle on steep grade and drove into sump, overturning vehicle to avoid other traffic. Investigation on-going as to reason for loss of vehicle control.
Injured was scooping in the #2 entry, rt. rib. The scoop blade was back under the curtain on the rt. side of the scoop. When he backed up, the curtain hooked on the scoop, he was attempting to free the curtain while backing out and caught his hand between the scoop and the rib.
Employee was operating MBC when he experienced pain in back. Employee continued working until January 8, 2007 at which time he started missing work due to this accident now making this a lost time accident.
Employee was riding mantrip to section when a rock came in the mantrip and struck his left leg below the knee.
Employee stated that while operating a Dozer, the dozer slid off a rock and jarred his back. Employee stated that he was wearing a seat belt.
EE sustained a hernia while stretching over a cross-beam and pulling on a wrench to loosen a hydraulic hose. He felt a pain in the abdomen and it was still hurting the next day, so he was sent to the Dr. The condition did not improve any and surgery was required to correct it. First day of lost time was 12/27/06.
EE was welding on coal broom. A piece of metal got into his right eye.
Contract laborer was assisting to change electric motor on shaker. When first bolt was loosened, the motor rocked back on the mount and smashed his left index finger, causing fracture and laceration requiring stitches.
Moving large rocks with dozer strained lower back - did not seek medical treatment until 1/3/07.
Employee was getting a crib block out of the belt head, cut right ring finger and strained right arm.
Employee was filling a lift truck with propane. Once the tank was full, the bleeder valve was frozen open. Employee tried to close the valve and sustained a chemical burn to fingers on the right hand.
Walking along belt and stepped in a hole.
Hoisting belt drive boom in the air when the chain of hoist slipped. The boom fell from about 3 feet and struck victim's left leg. Accident happened at #3 belt drive between #3 and #4 entries.
The day before accident Quincy area received approximately 3 inches of snow. As the employee was walking from maintenance shop to the control room using a paved roadway on south side of plant, he slipped and fell twisting his right leg causing a fracture of the right femur.
Employee was attempting to move rocks that had rolled under the haul truck due to spillage while loading. He experienced pain in his right knee.
EE went up on deck to check carbon proportioner and clean the area. Coming back down the ladder EE let go of it a few feet from the bottom thinking EE was at the bottom and fell backwards onto concrete floor and on left arm, causing it to break above the elbow. Transport to hospital was via ambulance and corrective surgery will be performed on 12/29/2006.
While testing the functions of the MRS, the injured EE had his left hand on the machine when a sudden movement caused a lid that had been raised to fall mashing the #3, #4, #5 fingers on his left hand, resulting in fractures and lacerations to these fingers.
Employee was walking to the bunker. He tripped on the track and fell and twisted left knee. Lost work days began 12/26/06.
I/E was tying off mt barge w/ 1.5"" line to winch cable, cable broke, pulling him down.
Employee was walking near a work area where track pads were being removed from an excavator when he was hit near his right eye by a flying nut. Employee was wearing his personal sun glasses and was not wearing his Company issue safety glasses at the time of the accident. He sustained injury to his eye. He was taken to the hospital emergency room for treatment.
Employee stepped on a piece of coal injuring his right ankle.
Employee was assigned to the 570 HT was performing a pre-op inspection at the start of graveyard shift. After checking the fire supression system on the deck of the haul truck, the employee turned and fell.
Operating North West Dragline. Pulled back on draw lever straining lower back.
Another mechanic was climbing up steps on rock truck, got to the top step, hand slipped off handrail causing him to fall on top of employee who was checking air leak, knocking him to the ground. Employee went to doctor on 12-28-06 and was released back to work. Started missing on 04-02-07.
While tramming scoop, employee got too close to rib & bumped rib with canopy of scoop. A few small rocks fell on his neck & shoulder. He stated that he thought larger pieces were about to fall. He jerked forward to avoid anything else striking him. He stated that he felt pain in his neck & shoulder after that.
EE slipped and fell on drill bench injuring lower back. There were no witnesses to the accident.
Employee was in the process of installing a 6' fully grouted roof bolt when he inadvertently hit the rotation lever on the roof bolting machine and caused the pin to become dislodged from the chuck on the bolter and strike him in the left wrist. Causing a fracture to his left wrist.
He was a passenger in MV-12 (Ford Pick-up Truck), operator lost control and vehicle hit rib.
EE stated he was pulling water hose onto dozer. The hose got hung up on blade causing his right little finger to bend back. The ee continued to work his regular duties with intermittent trauma causing discomfort & swelling. He has been diagnosed with inflamation in his wrist & little finger. X-rays show no fractures or dislocations. He was taken off work 2/5/07.
Employee was setting a hydraulic tailpiece jack at the section tailpiece. He alledged that a piece of rock broke loose from the roof when pressure was applied to the jack, stiking him on the left shoulder.
Employee stated he was unloading supplies off flat car with scoop, had hand over top of scoop canopy, raised scoop up with bucket, caught hand between scoop and mine roof.
Employee alleges his right shoulder started hurting while shoveling sand from beneath a conveyor.
Mantrip moving from parking area to staging area caught charging cable and tripped employee backwards, twisting his ankle and landed all his weight down on his foot.
Employee was initiating a bank push of the panline at #4 shield, E-15 longwall section. The #3 shield slid back causing strain on the high pressure shield to shield hose, resulting in the manifold breaking off and he was struck in the left thigh by the 1 1/2"" hose and coupler. The ee sustained a fracture to the left femur of the leg.
Employee was located on the offside of the Fletcher Roof Ranger II when a piece of rock fell from between the last row of bolts & the ATRS. The rock slid down the canopy & struck the EE on the right leg. Rock measured 18""X 14""X 4"".
Operator got something into eye while operating a dozer on the valley fill area.
Employee reported a twisted knee on 12/20/2006 when he slipped on the ice. Injury considered a strain requiring no treatment at that time. On March 1, 2007 employee went off work for right knee surgery. Physican related knee problem to strain injury on 12/20/2006.
Driver was releasing the roll bar on trailer #DT180. The tarp broke loose and spun the roll bar too quickly causing the operator to have to let go of the bar, resulting in right shoulder blade (scapular) strain.
Victim was walking down steps of bath house and slipped and fell on his right hip
Welding was being done on drill. Operator got flash burns to eyes.
Employee injured right knee when walking on uneven ground along side a jeep. After further investigation it was determined that this would be a lost time accident.
Employee sprained ankle stepping from locomotive. Lost time injury. Returned to work 12-28-06.
EE was walking on the facility dock when he slipped and fell. He broke his fall by reaching out with his left arm. EE felt a pain in his left shoulder. On 04/27/2007, EE had surgery on his left shoulder. EE is currently losing time.
Excavator was cleaning out around tail roll on product belt, striking ground wire with bucket. EE tried to signal the operator that the wire was hooked. He tripped and fell as the wire came tight and pulled over the control box, which is fastened to a stand, striking EE and injuring his ankle and foot. The control box and stand weight approximately 80 lbs.
Ee was adding hydraulic oil to roof bolter when he felt a sharp pain in his back.
Sprained ankle when getting off dozer. Tripped on rock and lost footing.
Employee let his boom down to walk drill out for service. He forgot to turn carousel to lock the steel in place. Starting back to work he raised boom and steel fell out. He was trying to roll the steel over into the rack with a bar. Steel rolled and the bar slipped & he went off side of drill onto the ground. Landed on side on his face.
Descending ladder - foot slipped off ladder rung, hit knee on ladder. Accident occured on 12/19/06 -did not seek medical attention until 1/24/07.
Employee was performing his normal roof bolting operations by installing a 5' roof bolt. As the employee began to install the bolt the bolt turned and he felt a pain and a pop in his wrist. Employee continued working until being diagnosed with a fracture to his wrist. First day away from work was 1-31-07
Was bending over to pick-up a tire chock, when he felt a ""tingle"" in his left eye.
HIT LEFT HAND MIDDLE FINGER WITH HAMMER WHILE SPLITTING ROCK
EE's by-passed safety devices on a raised cab, which created an unsafe act. Additionally, the injured EE attempted a single man lift where a team lift is applicable.
Rib rolled onto right calf muscle while bolting.
EE was walking along side the 374 belt to reset the alignment switch, when he slipped and fell.
EE was using the Vac Truck in the apron. As he was pulling the hose around the tail wheel, the hose went into a pile snapping his wrist and thumb forward. He had a hold of the stick attached to the hose. ***This was not reportable until 05/30/07 when he had surgery.***
(LTA) : Unit # 1 - Employee was lifting rub rails on #55 miner. Strained right chest muscle. Did not start losing time until 12/20/06.
EMPLOYEE STATED THAT HE WAS PICKING UP A FOOT SHAFT & STRAINED HIS LOW BACK.
Operator was cleaning loose rock from highwall when a rock came over his bucket and through the windshield
While lowering drill head with right hand on the hydraulic controls and holding a section of drill steel in left hand, the upper end of the steel came in contact with the side of the drill head and the lower end of the steel contacted the operator's foot. As the head was lowered it pushed the section of steel against his foot.
The injured employee was positioned alongside the roof bolting machine operator's deck and had finished conversing with the operator who was in the deck. The operator started the machine and the injured employee was caught between the deck of the machine and the rib.
He was moving a 1000 pound piece of steel when the cables lost tension and it fell onto his right leg. Broken lower right leg.
offloading 12' of 12"" channel from boat to barge, set one end down on barge, let other end fall to barge, placed foot on end that was on barge to steady, channel bounced and flexed foot toward shin
Employee was climbing down the steps of the quarry truck. When leaving the last step he jumped and did not let go of the hand rail and pulled a muscle in his arm.
While pulling cable, employee felt pain in lower right leg.
MM Establishes that on the morning of 12/16/06, at approx 3:30 am two ee engaged in horseplay which resulted in physical altercaution. There were a total of three ee on the shift. Due to minor conflicts concerning the statement provided by the injured ee, this incident is under investgation.
Hanging bottom roller, had bracket on one side while crawing under belt. Bracket came off, striking him on side of knee.
The EE had his hand outside the canopy and caught his hand between the canopy and rib. Laceration on left index finger, 20 stitches to close the wound.
Slab of frozen gravel fell out of back of work area striking employee about the knee area of the right leg; breaking lower leg in two places causing compound fractures in both breaks. The top of the foot was also broken, and the entire portion of the right lower leg badly bruised.
Stepped on walk board 3rd south submain intake overcast. Walk board broke. Employee broke through board and sprained his right ankle.
Employee stated that continuous operation of a front-end loader and bouncing in the seat caused twisting and strain of the lower back.
EE fell @ belt head. He didn't trip- just felt pain in his back.
Injured was walking from the track entry over to the belt starter at slope bottom. Injured struck his head against a truss bolt resulting in a stiff neck.
Employee was pulling 9ft steel with drill, strained lower back.
He was carrying a return roller with another employee when he stepped on uneven bottom & twisted his right knee causing a partial tear of the medial miniscus.
Employee was carrying drill foot clamp up a set of stairs, lost his balance, tried to recover and subsequently experienced a hernia. Treatment was not rendered until 1/2/2007.*Surgery to Repair*

Cutting flashing belt for under jaw conveyor/hopper with his right hand when the knife slipped and cut 2 fingers on his left hand.
Employee bent over laying out the sump bottom, when he stood up, felt sharp pain in his back.
Wind blew board against leg as he was unloading waste lumber.
As he was leaving office slipped & fell down steps & hurt back.
EE was walking across coal yard at surface prep plant, when he caught his foot and tripped and fell.
He was walking along the #3 entry to get a scoop when he tripped on a metal band & fell to the bottom. He sustained a contusion & strain of his right knee & shoulder.
Roof bolter was installing the second bolt of his first row when the roof inby the ATRS beam fell without warning. A piece of rock flew up striking his leg above the knee. Cause of this injury was slickenside material falling without warning. Compliance with rules/regs was not a factor, mining equip/system not a factor, miner proficiency maybe a factor, personnel equip ok.
Employee was walking across muck pile, rock under his foot gave way, causing his knee to give inward. Twisted knee.
Laborer fell backwards, stumbled and fell at grade while pouring concrete.
Employee reported: He & a co-worker were servicing the clean coal silo feeders. He climbed the ladder to the walkway to the feeder & proceeded to grease the north side bearing. When stepping around the end of the feeder walkway, he lost his footing, falling to the top of the belt cover then down to the load out tunnel floor. The backside of his head hit the floor.
As employee stepped down from the bumper of his mechanic's truck (a distance of approximately 24""), he stepped on a rock causing his left knee to give. He drove himself to the hospital where he was diagnosed as having possible ligament damage.
EE was cleaning mud from the 30"" stockpile chute at the raw plant when he said he pulled a muscle in his lower back.
The employee was working in a washplant where there was welding being conduted by another employee. The flash must have bounced off the walls in the washplant and flash burned his eyes.
Grinding a piece of metal with safety glasses on and some got in his eye.
He was standing outside of the shuttle car when a rib roll occurred and hit his leg against the deck of the car, resulting in a strain to the right knee. This occurred on the 003-0 section at the section feeder.
Installing #9 Rebar when foot slipped down into the formwork and rebar at the end.
Constructing an intake stopping, working on his knees, he turned to pull an 8 lb. bucket of plaster over to the stopping and experienced pain to his low back region. This was reported to his supervisor the following day. Approximately 33 days later, he reported he had a hernia to his groin area. Started missing workdays 1/16/07.
While recovering belt structure, he was lifting a piece of stand & pulled a muscle in his lower back.
Injured EE was cutting 18' drill in half to put in loader bucket to move to the right surface. He threw a 9' cut piece into a pile he had started and this piece bounced back and struck his right foot.
Employee had dismounted a bull dozer and slipped on some frosty grass falling on his right side.
Two employees were removing a metal liner from the 1 crusher discharge conveyor. While bringing the liner down to the catwalk one of the employees lost his grip which caused the second employee to be struck on top of the right foot.
Employee stepped up on scoop to fill duster and slipped and fell off. Hurt low back and neck. EE lost time for one day on 12/16/06 due to intense pain.
The man was walking up the entry, when his right knee gave way and twisted.
Employee was assisting to dislodge a rock in the feeder. The rock moved, striking EE's right pointer finger which required surgery & sutures to reset his finger. Accident occurred at section dumping point.

Employee was operating a 230 cat hauler and ran out of the road into the ditch.
Employee was handing a bucket of slag/dirt to a mechanic outside process fan and felt a pain in his back.
Employee was helping mechanic lift a pinner pot when he felt a pain in his lower back. He finished his shift on Friday, went to the doctor on Friday, Saturday and Monday and returned to work Monday night for his Tuesday shift.
EE was carrying a ladder across the parking area when he slipped on the ice, causing him to fall and twist his ankle. He went to have surgery on his ankle 4-11-07, this is the date lost time will start.
Member was pushing refuse material with the dozer at the impoundment and he had pushed material from the belt to an area where the refuse material was being loaded out from ealier in the day. The area he was pushing to had a drop off of about 13' where he had pushed too far and the dozer went nose first, causing his face to make contact with radio, causing 2 teeth to dislodge.
While unwinding grease hose from hose reel, employee felt a burning sensation in his abdomen, thought he had just pulled a muscle, however the pain persisted. The following day he told his supervisor. The employee went to the doctor, who took him off work due to a possible hernia and was refered to a specialist.
Employee stated that he was riding 3-wheeler mantrip to #6 head when he decided to cross the track in a place he says he probably wasn't supposed to cross. He stated that he had to go really fast to cross there and when he hit the track it caused his head to bump top.
EE was exiting a 10 sc shuttle car and lost his footing and went to catch himself and injured right hand.
Employee was walking down stairs in K3 tower and pulled back muscle
Slipped and fell onto side of body, against machine.
Employee stated that he was walking across section, slipped on loose material, went to doctor on 1-4-07. Continued to work until 3-20-07 when doctor removed him off work.
Employee was checking holes where he was drilling when he slipped on a rock and fell to his right, falling on an old broken steel pipe. Upon falling he cut the outside right side of his knee.
Employee was walking on slippery surface, rolled his ankle and felt intense pain. X rays were negative for fracture, employee was given a brace and released to full duty, however was unable to return to work for two shifts due to pain in ankle.
Employee was loading a structure trailer when it slipped off of block and caught his left foot.

Employee was cleaning vent lines on the roll press structure using a 5 lb sledge hammer. He missed the line and over-extended his elbow.
Operator backed his hauler through the berm, overturned and fell approximately 40 feet. He sustained a cut on his hand requiring stitches.
Was stacking 50lb bags of mine safety dust, had two pallets stuck together, one fell and he was turning to lay a 50lb bag down and he fell over the pallet.
While driving along haul road a piece of gravel flew from tire, hit the mirror and busted the glass. A piece of glass hit the driver in the eye.
Employee was repairing a drill, picked up a solenoid resulting in pulling his back.
While bolting top a piece of rock fell on atrs then fell in on victim hitting him on the shoulder.
Prior to the start of his shift, employee was walking down ramp toward change room when he slipped twisting his knee. Employee did not seek medical attention initially, but later requested to see a doctor. His knee was drained of fluid and is currently off work.
Employee says he was prying on a drive sprocket and felt a slight pull in groin area. This was not reported until 2/28/07. He says he did not report it as he thought it amounted to nothing. His Dr wanted him to cease activity for 2 weeks as he had a previous hernia repair in the proximity of his soreness.
Emp. was operating a bridge when his light got into the roof and it bent his head backward causing a strain to his neck.
Setting down concrete block, cabling came off spool.
Employee was rebuilding ventilation brattice in b-1 area when a piece of rib rolled off and struck him on the foot and ankle.
Electrical burn. Flash fire burns to the miners face and neck. Power was off, he opened the box to check the fuses and when he went to touch the fuse, it blew up causing a flash fire.
Untieing line before tension was released. Line released and knocked individual down resulting in accute sprain to lower back.
Employee stated that he was delivering parts to the job site in the parts truck. The truck hit a pothole in the haul road causing the employee to be jolted in the truck. He said he felt pain in lower back area and down his right leg.
Was under the bulk truck releasing the air valve on the air tank & while getting out from under the truck hurt back.
Employee was stepping off machine and twisted ankle
Employee was building stopping when the stopping fell over and cinder block struck him in the side of neck and back.
The employee was disassembling a truss, a sleaving bar was inserted to pry the sections apart. When the sections seperated, the bar forced the employee's left hand into the steel structure.
Operator stepped out of control trailer & said he was hurrying & missed a step & fell. When falling he missed the handrail & hit his back on the stair step.
While tramming motor by motorbarn to get supplies loaded, motor struck a broken piece of track and employee was jarred inside operator's deck, causing his head to hit the canopy resulting in a fracture to transverse process C-2 vertebra in neck.
Operator was checking for an oil leak, walking on the machine he stepped in oil & slipped. He fell approx. 5'to the ground, breaking his hip & pelvis bone.
Individual was helping lift an employee who had fallen resulting in an acute lumbosacral strain.
Working on cement line palletizer. Forearm struck by palletizer door.
The injured was roof bolting with a jackleg. He started to collar a hole and a piece fell 8 feet from the roof. He had looked down at the water valve and the piece hit the steel, breaking up. One of the pieces hit the brim of his hard hat, driving it into his glasses.
Employee was going through mandoor on g5 calline, slipped and twisted ankle. began missing work 12/15/06.
Employee was using a 5/8 high strength chain hooked to his shuttle car to try to pull a continuous miner. As the shuttle car started pulling on the miner the chain broke. The chain whipped around hitting the employee on the left side of the face above the eye and right cheek causing lacerations and a fracture of the cheek.
Accident is under investigation (EE slipped and hurt his back setting up a rail car).
During the raising process the workdeck winches continued to run and chains broke causing the workdeck to yo-yo. EE was injured on workdeck but did not fall off.

Employee was pushing on a piece of pipe trying to get bolts to line up with the coupling - felt a sharp pain in his upper right arm & shoulder. Went to doctor on 4-12-07. Started losing time on 4-13-07.
He was pushing an 8 foot combination bolt into the hole, as he was pushing the bolt, the bolt caught a separation in the roof and he felt pain in his back.
Employee walked between line to hang halo spray bar. He started to rehang halo spray. When entering spot to reach chain, caught right boot toe on toe plate. Felt some knee pain but no medical attention needed at that time.
EMPLOYEE STATES WHILE STANDING ON ROOF BOLTER, MUDSTONE FELL FROM ROOF IN BETWEEN SCREEN, STRIKING HIM ON HEAD AND SHOULDER.
Bent over to get dust box sealed, drawrock fell hitting him in the lower back.
While arc-cutting sticks on shovel, employee's pant leg caught on fire, burning his lower part of his right leg.
Employee was attempting to change a rim liner bolt on the Gyratory crusher. Employee had accessed the crusher dump pocket and slipped on loose material located on the rim liner. The Employee fell into the crusher and sustained a fracture to the Tibia Plateau. Employee was utilizing fall protection equipment to perform the task.
While shoveling belt line the employee twisted his back while emptying shovel onto belt line. Upon investigation of the incident and completion of subsequest medical tests/examinations, it has been now been determined that this is a reportable injury.
A mechanic lit diesel fuel in 5 gallon bucket to thaw air tanks on truck. EE was trying to stay warm by the fire and his clothes caught fire. He had on clothes from previous day that had diesel fuel on them. Contrary to company policy, the mechanic lit the fire. Air brake alcohol was provided by the company to thaw the lines out. The mechanic had instructed EE to watch the fire.
The ee was hooking a chain to pull a cable sled when the sled shifted in scoop bucket and caught the injured rt hand between the sled and scoop bucket.
The employee was assisting changing a shuttle car tire. The lug bolts were removed and the tire suddenly came off and rolled, striking the employee's right foot and lower leg. X-ray was negative, bruise.
Employee had come in early. He was traveling to section on the mantrip. When going around a 45 degree curve, visibility was limited. He did not see rock duster coming out and hit rock duster causing him to hit his ankle on mantrip.
The employee was attempting to direct the flow of concrete during a free-flow pour. The concrete level rose higher than his boot tops. This caused burning of the skin around both calves. The concrete also contacted his skin directly above the knees on both legs causing further burning of the skin. No medical attention needed.
Employee was getting off of motor when he twisted left knee (He did not start missing time until 12/14/06).
Bumped head on roof of mine while filling drill holes with explosives
EE was setting a timber and as he was reaching for a wedge, the timber fell striking his hammer which flew up and hit him in the mouth. This shattered his left bicuspid and split a small place in his lower lip.
Empoyee parked haul truck inproperly causing ladder to be higher from ground. EE got off haul truck he felt a pull in his shoulder. He did not feel anything was wrong and did not want to see a dr. He went to dr. 12/12/06 for unrated reason & told dr about shoulder. X-rayed found nothing wrong. still bother him return to dr. if to going for surgery. 4/18/07 he went for medical
Victim was putting supplies on bolter when a piece of draw slate fell between the bolts. 2'x3'x3"" striking victim in the head and shoulder.
After making a visual inspection, the employee decided to pull a piece of draw rock for the other operator. The employee was using an 8 foot long slate bar, the rock fell and the end of the slate bar caught on the T-bar, which caused the end that he was holding in his hands to strike him in the nose.
Employee picked up a chunk of material, dropped the chunk, it hit a shovel. The shovel handle popped up and hit employee in the left jaw.
Employee tripped over door threshold - not carrying anything - slip resistant footwear was being worn - acute knee sprain to right knee. EE given 7 days off by MD and vicodin. Rotational scheduled return date 12/28/06.
He was standing on the ground, prying frozen chunks out of the box, when one chunk of frozen dirt hit him on his left ankle.

He was pulling and lifting a pipe and felt pain in his left shoulder, he worked at his regular occupation but continued to have pain. He was eventually removed from work with a left shoulder strain on February 28, 2007.
Employee lifted a 5'I-beam for support of crusher & strained his back. EE initially didn't think he was injured, later he experienced stiffing & extreme pain.
The victim was carrying an insulator post for a shuttle car. He tripped over a trailer tongue causing him to stumble. He did not fall but the strain from trying not to fall while carrying a load resulted in a hernia. He began missing work on 12/8/06
Employee was putting discharge line in entry. Went to move pump that was frozen to ground. Broke pump loose and moved straining lower back.
While loading copper cathode onto 4 stripping machine receiving conveyor, employee had one copper cathode fall from cathode strongback and hit thumb on left hand causing an avulsion to left thumb.
Employee standing on catwalk when catwalk fell off the machine he was operating. This caused him to fall and he received 3 compression fractures to his lower back.
Digging out crusher, was kneeling and knee went out. He put knee in place and continued working. His initial Dr. Exam was on 12/11/06. He continued working until 2/27/07 when he had surgery on knee.
Employee was assisting with exhaust on tractor #67 and exhaust fell hitting his right hand.

EE was assisting the welder by helping to move parts out of the way or into place, lifting with a crane to make room for the welder to work, etc., throughout the afternoon of 12/5. Reflection from the welding caused welders flash.
Man was riding in mantrip; mantrip rubbed against rib and caught his lamp pouch, pulled man around straining his right shoulder. He started missing 3-30-07.
Greasing crushing equipment, he stepped on a rock and twisted left knee, tearing ligaments in the knee.

Employee was climbing back onto his CAT 785 rock truck when he slipped from the 2nd or 3rd step above the bumper (approx. 8' or 10' off of the ground) striking his face and right shoulder against the truck then striking the ground.
Miner was walking down a conveyor passageway, apparently he didn't notice the end of the conveyor with a ladder to step down, he kept walking and fell approximatley 5 feet to the ground.
Employee was building a road through shot to highwall. Road wasn't wide enough for dozer. Lower side gave way and dozer turned over.
Employee notified company on 12/14/06 that he injured his back on 12/5/06 climbing on or off a D9L bulldozer. He went to doctor and had an MRI done. Test show ruptured disk in lower back. Doctor is saying surgery is necessary.
Centers was helping to position the canopy on the roof bolter to align the bolts. During this procedure he strained his back. He was transported to Williamson Memorial Hospital by ambulance.
Equipment operator was holding onto the steering wheel of his grader when the blade struck a fixed object. The steering wheel turned suddenly and twisted the operators wrist.
Large rock fell from rib, hit employee on right shoulder.

Employee states he strained his low back removing belt structure from the LW belt entry.
Employee was using a rockbar to slide off a shuttle car tire that was on a supply car. When the tire was at the edge of the car it moved suddenly and the bar struck him on the side of his face. Cause was safety awareness. Compliance with rules/regs not a factor, protective equip. not a factor, miner proficiency a factor, mining equip/systems not a factor.
Employee was operating Volvo L180 loader. He was preparing to dump the 68th load into 17 belt hopper. Just before he tilted the bucket to dump into the hopper the shaft nut assembly on the dump cylinder came apart and the bucket rolled back. The material came through the front windshield into the cab. The force pushed the steering wheel down on top of both legs above knee.
Employee was cleaning pump, x-cut 7 of SME. His foot got stuck in the mud, he twisted his knee when trying to get free.
Employee lifting shrink wrap roll onto machine and wrenched back.
Employee was carrying bags of rock dust while kneeling over, when he felt his back pop and pain shooting through both legs.
Employee was loading resin on top of roof bolter when he felt a sharp pain in his lower back.
The injured employee was helping to finish the belt move. He was swinging a timber to knock in a bottom belt stand and twisted his lower back resulting in a bulged disc.
Employee was observing other employee check grinder from about 8 feet away. When grinder powered up, wheel came apart. Fragment from wheel hit employee in left ear.
Employee was disassembling and moving metal framed building. He turned to move away from the frame to lay it down. Fell to the ground in pain.
Empoyee lifted a heavy battery out of mantrip with poor lifting posture. This resulted in a hernia to lower abodmon.
Mechanic was performing service work on machine. While pulling on hydrolic hose, he pulled a muscle in his right shoulder.
While watching over his crew perform various welding, cutting, and using the plasma ark through out the day, he injured his eyes.
Employee was pulling slack out of miner cable feeling sharp pain in lower part of his back. Went to doctor on 12-4-06, continued to work, went back to doctor on 12-11-06 and started losing time on 12-12-06.
Employee was walking down belt slope and slipped on ice. He fell landing on back.
Employee got right middle finger pinched in the door to the crusher shack
On 12/4/06, EE was operating the shuttle car, tramming from #5 entry to #5 left. EE turned the corner into #5 left and the shuttle car slid into the rib because of the grade of the bottom. The canopy of the shuttle car struck the rib, allowing a piece of coal to enter the operators car.
CABLE HOOK & METAL BROKE OFF OF MINER & STRUCK EMPLOYEE IN RIGHT LEG.
The employee and the off-side roof bolter pulled the roof bolter machine into # 3 room to roof bolt a cut. The injured employee stepped over to the hand controls to position the machine, when a piece of rock 60"" x 22"" x 0-4"" fell out of the roof next to the rib line. The rock struck the employee on the head knocking him into the bolter or the floor.
Employee was using a belt knife to cut open a cable, when the knife slipped, cutting his left thumb.
Employee was attempting to strike the overflow barrel of the cone with a mall to break up ice that had accumulated over the weekend. When he swung the mall backward, his foot slipped, he lost his balance and fell over the handrail to the ground resulting in his injury.
EMPLOYEE CHECKED OIL IN DOZER (D-9). HE WENT TO GET INTO CAB AND STRAINED HIS BACK WHEN HE TURNED.

EMPLOYEE WAS RUNNING MINER, STEPPED BACK AND STEPPED ON A ROCK, SLIPPED AND POPPED HIS RIGHT KNEE OUT OF SOCKET.
Employee was raising the belt up, using a jack when he felt a pain in his right shoulder. Employee continued to work and continued to have pain in his shoulder. He went for an MRI and had positive results on 4/26/07. He began missing time on 4/30/07 due to surgery on his right shoulder.
EE was operating his dozer on layers of rock when one of the upper layers broke causing his dozer to drop sudden resulting in a neck and back injury. This injury occurred on 12/2/06 and he started missing work on 12/20/06.
Was laying a bolter cable over against the rib when the employee hit his head on the brow.
Operator was climbing the ladder to the cab of a haul truck when he slipped and fell. Employee had been working with the lowboy, handling the chains and binders. Operator commented that his gloves were slippery due to the lowboy chains and binders. Employee was diagnosed with a bruised tailbone.
EE was moving the mule cable out of the roadway, when he stepped in a hole, twisting his right knee. His 1st day away from work will be 1/2/07. He had surgery to repair a torn ACL in his knee on 12/22/06 and was on a scheduled vacation until 1/2/07. He was unable to return to work after the surgery.
Employee was helping hang an anchor for a shuttle car cable when he felt a pulling or burning in his neck.
The employee was exiting a skidloader and his foot got caught and he fell sideways hurting his foot and maybe his toe.
The employee was cutting a piece of belt with a knife, the knife slipped cutting his left hand thumb.
Employee was lifting a hydraulic ram and strained back. He was working on dozer at prep plant.
EE was operating a continuous miner when a piece of roof rock fell out between the rib and rib bolts. The rock was approx. 7'L,2'W, 0-5"" thick. The rock struck the back of his head and upper back. It drove him down onto the mine floor and he was partially pinned down by the rock being hung on his mine bolt. He receved abrasions and contusion to his back and leg.
EE was helping splice belt and usually a piece of diesel equipment was out of service. EE got a golf cart to use for transportation. While traveling down inclined travelway, cable broke on braking system. EE jumped off golf cart and bruised his left hip.
Employee was operating a continuous miner, changing places in number 4 when a lump of coal came off rib and caught his right knee between rib rail and rib resulting in employee being taken off work until further medical testing is completed.
Ride hit a bump in the roadway. EE was bounced up and came down hard on the seat. At 3:30 am EE was complaining of back pain and pain in the left leg. He then called out.
Employee was attempting to change the middle roller of the top belt. He locked & tagged the belt out & proceeded to place his body between the top & bottom belt to take the weight off of the roller. He heard a pop in his lower back & experienced sharp pains shooting into his legs.
Employee claims to have injured his back while picking up flights for a conveyor chain. He did not start missing until 2/8/2007.
After installing the right outside bolt the employee was swinging his drill boom in to put the 2nd bolt in. While starting to swing a piece of rock measuring 3""thick X 2'long X 18""wide came between the bolt & rib hitting the drillers canopy, breaking into pieces, one of which dropped to the mine floor & struck the EE on the left ankle.
While walking down some stairs, the employee's right knee locked up causing him to fall forward and in the process of catching himself twisted his back.
Employee took parts to the Galatia North Portal. While using the forklift to put the parts on the cage, he tried to push the door open further while still on the forklift. The forklift rolled back and caught employee's left arm between the forklift canopy and door frame.
Employee fell from loader step striking head. ***Safety Manager notified our local MSHA Office in Las Vegas, NV within the designated time frame.
Climbing down ladder - thought he was on first step. Instead he was on second step. He fell and the pipe hit him in the ribs.
EE was holding up the stacker cable to allow the loader to clean underneath. When the loader backed out, the material caught the cable and gave him a quick jerk. He did not leave work or seek medical attention. He worked until 12:00 pm on 1-3-07 and at that time, requested medical attention. He returned to work the next day 1-4-07 on restricted duty.
Miner stood up from working on loader and hit head on the boom.
Employee was working on the classifier at the sand plant and he was coming down the catwalk and slipped and fell into the handrail striking his left side of his chest area. The tempture was freezing and it is possible some ice had developed on walkway.
Emplyee started drlling rib holes and outburst occurred.
Operator was in an A40 D Volvo truck while being loaded by a 988F Cat Loader. While the loader operator was preparing to dump the first bucket of rock in the truck bed a rock slipped out and struck the bed jarring the operator spraining his lower back.

As she was descending the HT ladder, the wind blew her hard hat off. She reached for it and slipped, her left arm catching all her body weight. She strained her arm and back, but continued to work. On 1/11/07, she was medically unfit to work.

A 17'4"" x 3' x 1/2"" thick liner plate was being repositioned in the Pioneer feeder. The 5/8"" grade 8 bolt holding the upper right corner in place sheered allowing the plate to fall to the pan. The plate struck the employee on the left foot.
The employee tripped over a large rock and fell, straining his right shoulder. The employee continued to work at his normal job until 1/22/07. He then went in for shoulder surgery and began missing time.

Employee was attempting to cut a zip tie loose with a utility knife. In doing so, the knife slipped cutting his left lower leg (calf). The laceration required six stitches.
A mechanic was lying down inside the 1st stage blunger tank at 1-C Cowles attempting to remove the bottom blade. After an unsuccessful attempt at removing the blade, the mechanic lost his balance causing his left hand to come intact with the blade. He sustained a laceration across the palm of his left hand.

EE started to kneel during her normal work duties when she felt her knee pop. There were no contibuting conditions other than a previous injury that the employee stated happened out of the workplace. Damage was to the right knee acl which will result in surgery.
A co-worker was pulling a drive chain back into position when the chain slipped out of his hands striking the injured worker in the right elbow.

Employee drove pick-up truck into drainage ditch. Driver was going too fast. Passenger scraped his hand and bruised his hip/back. Passenger was examined at the emergency room at the local hospital the next day and released. Emergency room doctor advised employee to take Monday off from work and consult personal physcian if necessary. Passenger EE returned to work Wed.
Upon an investigation, the EE alleges the RBM cable struck him across the head, knocking him down to the ground. The EE's complaints were the lower back and knee. EE was transported by ambulance to the ER where he was treated and released. He is on transitional duty with restrictions for a back and neck strain noted as subjective without spasm.
The employee stated he was riding in an open mantrip to the section. A roof bolt had fallen out of the roof and the mantrip struck the bolt causing it to glance and strike the employee on the left side of the chest.
Employee was removing tailgate on a Cat 773B Coal Truck, using a three part choker sling and the National Series 1100 crane. The choker was off center, so they set the tailgate back down on the truck bed to remove tension from the cables and readjust them. As the employee was adjusting the cable the load shifted pinching his right index finger between the load and the cable.
While sitting in his 100-ton CAT haul truck employee was reaching for his lunch pail while getting loaded. When the loader dumped the shot into the truck bed the truck shook violently causing the employee's shoulder to be jarred upwards while he was reaching for his lunch pail.
Employee was using a ratchet to loosen the nuts on the crusher cone when the ratchet snapped in half causing the employee to strike his hand against the cone causing a contusion to right wrist.
Employee was dismounting a dozer. As he stepped on the botton step on the push arm, his left foot slipped off of the step and he overextended his right leg. He was evaluated by a physician and was taken off of work.

Employee was pulling on cable, when the cable caught on a chunk and employee felt pain in his lower abdomen. Employee had hernia surgery on 2/15/07 and began missing work.

Upon energizing conveyor the stabilizer to the gear box collapsed allowing box to spin 35-40 degrees. The gear box struck employee knocking him off the ladder. When others found him he was laying flat on back; ladder was still in position.
Walking to break and the employee slipped on icy surface, he lost his balance & fell on his left knee.

EE doing routine inspection of fan house, on top of 19 d/l. at the end of house, last fan, on/ off cab side, he stepped off main floor to edge of house, on top of expanded metel guarding to remove a dead bird, when the guarding metal broke out of frame and employee fell 3 or 4 feet to roof of machine house and hit his right side in rib area.
While walking from Bldg 450 Jet Mill Bagging Area to Stencil table, employee slipped on wet floor with left foot. He was able to prevent himself from falling, however he did report pain in lower back and left shoulder.
