Roof fall near spad 8154 measuring 25' long x 18' wide x 54"" high. The fall occurred on the #1 Unit 002 Section. 4' resin bolts and 6' torque tension bolts are being used in this area. Fall was cleaned up.

A rock fall occured 2 breaks inby spad number 3176 in the crosscut between the number 4 & 5 entry and partly into the 5 entry. This was reported to MSHA at 9:30 PM. The fall was 60' long, 20' wide, and 5' thick.

A fall was discovered in the no. 5 intake entry of the 1 North marker set of entries. The fall measures approx. 35'long x 20'wide x 2'- 4'thick. There was no equipment involved, there were no injuries & ventilation was not affected. The fall will not be cleaned up.

An unintentional roof fall occurred at spad #16+36 on the 4th submain east, #mmu-002-0, #2 unit, #1 entry in return. The intersection had been previously cribbed out due to a slip condition, the adjacent crosscut fell riding over the cribbed intersection. The area was approx. 20'in length, 60'wide & 10-15'in height. The fall will be cleaned up at a later date.

Unintentional roof fall discoverd on abandoned #1 Section. Fall measured approximately 70'L X 18'W X 10'H. Area previously supported with 6' resin grouted bolts and 10' cable bolts. Fall was from #1 entry to #2 entry just left of station 4981. Fall was discovered during examination of intake and return airways.

An unplanned roof fall occurred 6500' from the active face in the #7 entry at spad #666. No injuries reported. No equipment involved. Passage of persons was not affected. Fall measured 50' length X 20' width X 6' height. This fall wil not be rehabilitated.

Unintentional roof fall on abandoned #1 Section. Fall measured 75'L X 18'W X 10H. Area supported with 6' resin grouted roof bolts and 10' cable bolts. Fall location: Sta. 4985 to sta. 5000. Fall was discovered while examining intake and return airways.

A roof fall was discovered in the No. 7 intake entry of the 1st North Submain of #2 unit. It was in an intersection at spad 27+09 (xc 39) and extended inby one half of a crosscut. It measured approximately 50 X 20 X 8' high and was about 3600' outby the section dumping point.

A roof fall occurred that blocked the tailgate at x-cut 19 in LW-203. The fall started in the x-cut between No. 1 and No. 2 entries and ran out in to the tailgate entry.

A roof fall occured at Break 45 of the North Submains, starting at the left hand rib line of the #3 enteryand proceeded through the crosscut to the #2 entry and throught the intersection of the #2 entry and stopped at the stopping wall between the #1 and#2 entries approximately 6 feet in height.

Unintentional roof fall occurred on 2nd northeast mains, #1 entry, 100' outby spad #2567. 30' long x 20' wide x 10' high. 2000' outby 003 section. Bolted with 6' torque tension bolts & 8' point anchore bolts in return air course. Found making weekly exam.

Fall exists at 83 break in 1 & 2 entries at spad #6904. Fall is approx 40' length, 19' wide and 6' thick at about 4000' to the face. 6' torque tension bolts were used in 1998 during development. Fall does not block airways or travel areas. It will be cleaned up after proper plans are made.

Five miners were in the Fish Creek elevator travelling into the mine when the elevator stopped. The miners were on the stopped elevator for 48 minutes.

A fire was discovered in the Primary Crusher stilling shed at 5:05 AM. The fire was extinguished at 6:30 AM. There were no injuries resulting from the fire. The ignition source of the fire could not be identified. Coal on the grizzly and rubber belts hung to suppress airborne dust burned.

Examiner found a roof fall at the 88 Xcut(7081') to just inby 15' of 89 Xcut (7161') in #6 entry of the main north headings. Fall is approx 90-95' long X 7-8' high X 17-18' wide. Primary roof support was 6' tension rebar. Fall was timbered and cribbed on north end of fall. All approaches will be timbered.

A rock fall occurred int he #1 entry approx. 80' inby the #1 portal. The rock fall was 17' long, 18' wide, and 6' thick. Fall was approx. 1400' from the working section.

The fall was found at 2:00pm, while the section was idle. This report is in regards to the report of a roof fall. The fall was located in the #7 entry, two breaks inby spad #1998 of the 003 mmu. No equipment, ee or things of the like were injured. Also ventilation was not restricted either.

October 27, 2006, at 6:15 pm, an electical conductor to the #2 hoist slow down switch became disconnected. The mine evacuation started at 6:15 and was completed at 6:23 p.m. The hoist slow down switch wiring was repaired, the hoist tested and miners returned underground at 9:15 pm.

An unintentional roof fall occurred at break #17, #3 entry of east West Mains, 2 breaks outby spad #4052. The fall was approximately 35' long, 20' wide, 7 to 8 feet thick.

An unintentional roof fall occurred at the #3 set of seals in a crosscut between #15 and #16 seal to the left of spad #2007. The fall does not hinder the examination of the seals or block the regular travelway. The fall measured approximately 2' to 4' thick x 19' wide x 25' long.

Main portal elevator had an electrical problem and was not operating properly. The MG set dropped out and would not properly raise and lower the cage. Repairs were made and elevator was put back in service at 11:50 p.m.

Operator of remote loader was mucking out a longhole shot when the loader ran up the right rib and tipped over onto the driver's side. The loader caught fire. Mine was evacuated; evacuation started at 3:15 pm. All employees were on the surface at 4:45 pm. 103K order verbally given from MSHA and was terminated on 10/3/06.

At the New Era Portal, The slope hoist became disabled. Broken strands were found in the cable. Issue was corrected and hoist was placed back into operation 7:30 p.m. the same day

A roof fall occurred in the south sub main, entry #11, station #3801'. The fall was approximately 20' long, 20' wide, and 8' tall. The area was bolted with 6' & 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.

An unintentional roof fall occurred on the 1st south main xc 34 - xc 37 (20 + 40 - 21 + 60) in the #4 entry. The fall was approximately 130' long, 15' wide and 9' high. This area of the mine had been dangered off on 9/19/06 due to questionable roof.

The continuious miner was stuck in a pillar run and before the miner could be pulled out a planned fall occurred & caught on the miner. Mining was disrupted for more than one hour and is therefore reportable.

A roof fall occurred at 9:32 pm on Tuesday October 24, 2006 in the 2-east main belt entry. The roof fall is located between spads 10054 and 10068 and is approx 20' wide X 6' high and 70' long. This area was supported with 6' resin bolts at the time of the roof fall.

A fall was discovered in the No.4 Neutral Entry of No.3 Southwest Mains at Survey Station 1639. The fall measured approximately 60' long x 18' wide x 5' high and had been bolted with 4' fully grouted resin roof bolts. There were no injuries or equipment involved. The fall will not be cleaned up.

A roof fall occurred in the 2 Left Section. It occurred in the No. 4 Entry, 1 X-cut inby Station #254. Fall was approximately 6' - 8' high, 19' wide, 65' long. Fall caught high voltage cable along with the required lifeline. Area was timbered and dangered off. Escapeway was re-routed along with the required lifeline.

On the 002 section, in the #3 entry, one block inby spad # 1764, an intersection fell. 18'wide X 35'long X 7.5'high. The area was bolted with 3/4 X 72"" fully grouted bolts. The roof was laminated slate & sandstone. This fall occurred over an idle weekend. The area was dangered off & will not be cleaned up.

Roof fall (60' x 19' x 6' thick) occurred on #1 section in #4 entry, 1 break out-by spad #18898 in belt entry. MSHA notified by phone. No ventilation was affected.

The Tyro Creek elevator was down from 12:20 pm to 11:20 pm as a result of a power outage. When power was restored, Bagby elevator was called to make some adjustments & check-out the systems.

The page service shaft elevator went down approx. 7 am due to a brake pump motor. The service elevator was down approx. one hour which included repairing & testing. There was nobody in the elevator at the time of the incident. There were no injuries or damage to the elevator.

A roof fall was discovered approx 1200'-1500' from the faces of the 001-0 section. The fall occurred two breaks inby survey staion No.2874 and measured approx 70'-X 20' wide X 2' - 4' thick. The area had been bolted with 36"" fully grouted resin roof bolts and some cribs had been installed in the area.

A roof fall occurred on #2 unit inby the section loading point. The area had been previously detected prior to the fall and the area had been timbered and dangered out before the fall. The fall was located in entry #1 which is the far left return entry. The fall was located in the 2 Left - 5 West panel inby crosscut #15 and fell for a distance of approximatey 70 feet.

North 1, 19 - 20 block, # 7 entry, MMU 004 Spad 524. 18 ft wide X 25 ft long X 8 ft high. Support: 6 ft 3/4 inch resin bolts, pans, straps, 12 ft cable bolts, truss bolts and screen and ACS jacks on either side. Fall dangered off, additional roof support installed around fall.

A reportable roof fall was found in E-South Mains, entry 8, crosscut 10. The fall was approximately 7 feet high, 34 feet long and 34 feet wide.

An Excavator being operated by a contractor contacted a buried 13.8kv power cable. The electric breakers did not operate properly causing a fire in the substation and small fires in the cable trays in the mill. Some electrical switch gear was destroyed and power to the mine site was curtailed.

A roof fall occurred in #6 entry at #8 wall, 4way intersection. #1 North Mother Belt. The fall was above anchorage. There was no damage to equipment. The fall was 20'wide, 25'high & 54' in length.

Two men became stuck on the Livingston shaft elevator in the process of traveling out of the mine. The elevator stoppage occurred due to a faulty door switch. The door switch was repaired and the elevator returned to normal service at 9:10 pm.

An alledged frictional ignition occurred on the 7 Mains section, approx. 100 feet int he #5 entry. The operator was trimming his top in the b-cut and noticed an orange flash that lasted less than one second.

HOISTING INCIDENT : Cowling Portal Elevator - On Wednesday, 10/18/06 at 06:05 pm the Cowling Portal Elevator was taken down and was inoperatable for more than 30 minutes. MSHA was notified at 06:35 pm by Tamara Betancourt. The door switch fuse in the control panel had to be replaced. The elevator was operable and running on Wednesday, 10/18/06 at 10:05 pm.

A roof fall occurred on the South Mains ""A"" beltline at 69 x-cut. The fall is approx. 40 feet long by 18' wide and 9' high. Area was bolted with 8' Install 3 bolts and resioned. Other support that were installed were 4"" H-beams and cribs. Cause of this fall was deterioration of the main roof due to weathering.

The Tyro Creek elevator was down from 10:30 pm on 10/17/06 to 1 am on 10/18/06 due to insufficient airgap on brake causing the brakes to overheat. Bagby was called & corrected the problem, tested & placed back in service.

An unintentional roof fall occurred on the #3 conveyor beltline at 9:30 AM. The fall is one x-cut inby spad #2342. The fall is approximately 8'-10' thick x 30' long x 35' wide.

A roof fall occured on the 1st s.w. main #2 entry, between 0+70 and 1+40. The fall is 20' long, 19' wide, 10-12' high.

Material above the tailgate shields sloughed in around the tailgate drive and face conveyor. The Tailgate entry itself remained open in the active workings. The blockage was removed as the shearer cut out the tailgate on that same pass.

The fall was 50 ft long, 20 ft wide and 12 to 14 ft high. The fall will not be cleaned up. All appoaches have been cribed off.

A roof fall was discovered in the 2 east area of the mine on 4 right. The fall was approx. 40'long x 18' wide x 6' high. The fall will be breakered off and will not be cleaned up.

The final lift of a pillar was being mined when draw rock fell on the continuous mining machine fouling it in place. The incident occurred between the no. 6 & 7 entries of the 3-left off South Mains Panel (one break inby survey station 671). A retriever was used to free the machine - within 25 minutes after hook-up. The fall was inby the last row of bolts & no bolts were pulled.

The #1 elevator was out of service for a period exceeding thirty minutes. The brake battery back-up was inoperable. The system was reset and the back-up for the brake was operational.

Fall occurred in the track entry at spad #112. Fall was 20'L x 20'W x 8'H. Intersection was bolted with 4' resin bolts. Fall was approx 3400' from face. Men were evacuated and area has been timbered. off. A cleanup plan has been posted and is being followed. There were no injuries.

An unintentional fall occurred in the main north #8 entry at the 21+50', due to deterioration of the roof above the bolt anchorage. The area had been bolted with 6' tension rebar bolts.

Deterioration of immediate roof around roof bolts. The fall occured on our main north track at 85 cross cut. The fall filled an intersection and mieasured approx. 25 ft in length, 25 ft in width, and 8 ft high. The fall was bolted with 8 ft and 10 ft bolts before falling. It also contained 12 ft supplemental cable bolts.

An unintentional fall of roof occurred on 012 section while retreat mining operations were being conducted. The continuous miner and the inby bridge were covered by the fall which was app. 20"" x 70"" x 7'. The fall occurred in the #3 heading, break #19 at spad number 11131.

The elevator at the Hart street portal failed to operate. The elevator would not move up or down in the shaft. The cause of the malfunction was traced to a faulty elevator brake rope switch. The switch was replaced and the elevator was returned to service.

The electrical power to northern Taos County was lost during a storm. Our main hoist suffered damage to a rectifier that became evident about an hour later when 51 miners were coming to the surface at the end of their shift. Electricians troubleshot the hoist to find the problem and replaced the part. Hoist returned to normal ops at 5:40 pm and all miners came to the surface.

Section C-9. On 10/04/06, an unintentional roof fall occurred on the 42 south belt line #5 entry, 12 cross cut. This roof fall was supported with 6' point anchor roof bolts and 10' point anchor roof bolts. The roof fall measured 30' long X 30' wide X 12' high and the roof fall will be cleaned up and resupported.

A Taylor forklift parked by the hoist house drifted forward and ran into the hoist building knocking out the power to run the slope hoist. Equipment sat there for 1-1/2 hours before moving. Cause of this incident was not blocking wheels when vehicle is on a downhill slope and possibility of not setting the parking brake. Hoist was back into service around 4 pm.

A lead person was jamming an undercut rib for rehabilitation efforts in a secondary stope, he noticed trickling of rock from the right rib and backed out of the area. A ground fall of 25 feet long by 17 feet height by 6 feet wide came down from the sill to the middle of the arch.

Employee was observing the mechanic working on a shuttle car. It was revealed during the accident investigation, that the mechanic pressed the tram pedal, causing the shuttle car to inadvertently move. The shuttle car crushed the employee between the shuttle car and the rib.

Accident happened when a cut of coal was blasted in breast. From preliminary report: According to the witness, he heard three explosions. Two were normal. The concussion from the third explosion was excessive. The victim was eventually found at the bottom of #19 chute.

The miner operator on the east mains, left section, MMU 007-0, received fatal crushing injuries when portions of the right rib and corner fell unexpectedly. This occurred suddenly while the miner operator was cleaning the second cut in the 5 to 6 crosscut. The rib anomaly was approx 8.5' X 4.5' X 4.5' in the 5 to 6 crosscut and fell pinning operator against shuttle car.

Roof fall occurred in the outby area of the Old No. 8 Section. A rock fell onto the victim, causing injuries to the employee. The rock that fell onto the employee, was in the intersection at Spad 3575, in the old works of the mine.

16 yard Rohr Dredge located in lake on N Hummer Lake Rd in Oxford flipped over (cause unknown at this time). Employee drowned. Employee's body was recovered at 5:35 pm. Incident is under investigation by MSHA inspector Steve Field.

Victim was pronounced dead at the scene by the assistant Knox County Coroner.""

Miner was assisting two other mines in sliding a 2' X 6' X 1"" steel plate three feet away from dry grass. Miners were repositioning plate so that cutting sparks would not make contact with, and ignite, grass. The distal tip of the small finger on the miner's right hand was severed when he reflexively jerked his hand back when it made contact with a rock underneath the plate.

EE was running a hydraulic power cutter, got his hand caught in the hydraulic cylinder causing amputation of left thumb and mutilated his index finger.

This incident was to have occurred while this employee was waiting for the shuttle car to pass him. He was standing in a x-cut and did not notice that the snub was near him and he got flipped by the cable snub. Cause of this injury was safety awarness. Struck against stationary object

Employee was bolting in #2 right break when he bumped his knee on the ground. Employee started missing 10-06-06.

Struck against a moving object

Operator left property complaining of back strain and received medical treatment from Oct 6 thu 19, 2006 from chiropractic center. His report stated the jerking and jarring from a large rock caused pain.

Caught in, under or between (Not Elsewhere Classified)

Employee was moving a rock w/crane. The rock shifted in the strap and the ee lost control due to momentum. His right leg was pinned between the rock and the wall of the building immediate swelling and bruisng upon dr. visit, he was diagnosed w/torn tendons in his right shin/calf.

Caught in, under or between a moving and a stationary object

While backing up, he got his thumb caught between ROPS and guard on conveyor belt. Plant was not in operation.
