A ROOF FALL OCCURRED IN #5 ENTRY OF PEACHTREE MAINS. IT EXTENDED FROM #5 ENTRY OUTBY X-CUT TO 20+70' AND LEFT THROUGH THE INTERSECTION AND X-CUTOVER TO #4 ENTRY. THE FALL IS APPROX. 140' LONG X 18' WIDE X 10' TO 12' HIGH. THERE WERE NO INJURIES AND NO PROPERTY DAMAGE.
WEST MAINS 84 BLOCK #2 INTERSECTION. 7 FT HIGH X 50 FT CROSSCUT LENGTH X 40 FT HEADING LENGTH X 16 FT WIDE. TYPE OF SUPPORT: 6 FT RESIN BOLTS, 6"" X 16"" PLATES, ROOF MATS, CRIBS. ALL AREAS LEAD ING TO FALL DANGERED OFF AND CRIBBED.
THIS MINE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL OFF SECTION FOR MMU 014-0 IN #5 NEUTRAL ENTRY AT SS NO. 160 & EXTENDING INBY TO SURVEY SPAD NO. 161 APPROX. 120'. THIS AREA IS O N 14-E PANEL OUTBY THE SECTION. FALL WAS APPROX.120' LONG, 5' TO 6' THICK & 18' TO 20' WIDE. NO PROPERTY DAMAGE. PASSAGE WAS NOT IMPEDED NOR VENTILATION AFFECTED.
A ROOF FALL OCCURRED IN THE #3 RIGHT PANEL OFF SOUTH MAINS IN #6 ENTRY BEGINNING ONE CROSSCUT INBY SPAD NO. 36573. THE FALL WAS APPROX. 20' WIDEON ONE END REDUCING TO 15' ON THE INBY END. THE FALL WAS 60' LONG & APPROX. 5' HIGH. THE TOP WASBOLTED WITH 4' RESIN BOLTS. THIS AREA WAS ABANDONED 4/25.
THE SERVICE SHAFT ELEVATOR HOIST WENT DOWN AT APPROX. 10:00 PM WHEN THE POWER CABLE FOR THE ON-BOARD CONTROL BOX BROKE LOOSE FROM THE BOTTOM OF THE ELEVATOR. NO ONE WAS ON THE CAGE & NO INJURI ES RESULTED. CAGE WAS AT THE SURFACE LEVEL WHEN POWER WAS LOST.
THE ROOF FALL WAS FOUND AT SPAD#4037 ON 1 NORTH WEST SUB MAINS ON A WEEKLY RETURN AIR COURSE RUNTHE TIME OF FALL IS UNKNOWN. THE FALL IS APPROX.18'WIDE, 50'LONG, 8'HIGH.
A ROOF FALL OCCURRED IN C-2 MAINS, #3 ENTRY. THIS AREA WAS PREVIOUSLY MINED. NO MINING WAS BEING DONE IN THIS AREA.

ROOF FALL WAS DISCOVERED ON ROADWAY ON 1 RIGHT OF THE 001-MMU MAIN LINE AROUND 4:30 AM. FALL WAS TIMBERED OFF AND THE ROADWAY WAS MOVED TO ANOTHER ENTRY. ADDITIONAL BEAMS WERE INSTALLED IN BY AND OUT BY AREA AFFECTED. THIS DID NOT AFFECT VENTILATION.

THE BRAKE SOLENOID ON THE ROSS OVERWIND MAN CAGEGROUNDED WHICH DISABLED THE ROSS CAGE FROM 3:35 PM-8:00 PM. THESOLENOID WAS REPLACED. MSHA WAS NOTIFIED VIA PHONE/VOICE MAIL.

ROOF FALL IN FACE OF #3 ENTRY 20' WIDE 10'TO 12' HIGH 30' LONG ROOF IS SLATE TOP WITH PLACES IN IT THAT BROKEN UP.

A FALL OF THE ROOF OCCURRED AT X CUT #55 OF THE MAIN WEST, PILLOW LINE BETWEEN #1 AND #2 ENTRY. L-30' BY W 18' 10' TALL. TIME OF FALL IS UNKNOWN.
THE AREA OF ROOF BECAME VERY WET WITH THE RIDER AT 6'. 7800' UNDERGROUND OFF #6 MAIN RETURN IN #7 HEADING IN 6 LEFT MAIN. VENTILATION WAS AFFECTED.
EE 3RD SHIFT PLANT MECH. WAS CUTTING METAL WITH A TORCH IN A VESSEL ON THE 3RD FLOOR. TWO RUBBER MIXING LINES WERE BESIDE WHERE CUTTING WAS TAKING PLACE. A SMALL FIRE STARTED ON THE RUBBER MIX ING LINE AND EE GOT A FIRE EXTINGUISH TO EXTINGUISH THE FIRE AT THIS TIME AN IGNITION OCCURRED INT HE MIXING LINE ACETYLENE OR UNKNOWN GAS. THE FIRE WAS EXTINGUISHED AND THERE WAS NO INJURIES
THE ASEA PRODUCTION HOIST TRIPPED THE POWER TO RE-SET BUT THE POWER WOULD NOT STAY ON. HE NOTIFIED PERSONS IN THE MINE, THEY HAD ALREADY SHUT DOWN, IN PREPARATION FOR SHIFT CHANGE. HE CLALED T HE ELECTRICIANS AND OTHERS, AS OUTLINE IN THE EMERGENCY PROCEDURES. WHEN THE ELECTRICIANS ARRIVED THEY IDENTIFIED THE PROBLEM VERY QUIKCLY REPAIRS WERE COMPLETED, TEST RUN MADE AND HOIST WAS
A ROOF FALL OCCURRED AT 830 PM ON 4/26/01. AT SURVEY STATION 13 & 30 ENTRY 3-4, IN SLMS. MMU 006-0. THE FALL WAS 80' LONG 20' WIDE 12' HIGH. AREA WAS MINED BY 4-19-01. AND HAD BEEN SUPPORTED W ITH 6' FULL RESIN BOLTS. NO INJURY RESULTED BUT 100' OF HILINE WAS LOST.
UNINTENTIONAL ROOF FALL. FALL SIZE 56' BY 19' BY 6-7' TYPE OF SUPPORT: 5' MECHANICALLY ANCHORED RESIN ASSISTED. DEVELOPMENT DATE 4-25-01 FALL AT OR ABOVE ANCHORAGE POINT.
RESULTS OF NON DESTRUCTIVE ROPE TESTING, X-RAYS INDICATE HAUL ROPES NEED REPLACED, HOIST REMOVED FROM SERVICE AND ANOTHER ESCAPE FACILITY DESIGNATED AND ALL EMPLOYEES NOTIFIED.
A ROOF FALL OCCURRED IN #10 ENTRY OF THE 7TH EAST AT THE 4380 STATION. THE FALL WAS 20' LONG, 25' WIDE, AND 8' THICK. THE FALL DIDN'T AFFECT TRAVEL, VENTILATION, DAMAGE EQUIPMENT, OR CAUSE ANY INJURY. THE AREA HAD BEEN BOLTED WITH 6 AND 8 POINT ANCHOR BOLTS.
WEST MAINS #66 BLOCK # 3 HEADING OUT BY TAIL PIECE TYPE OF SUPPORT, 6 FT RESIN BOLTS 6"" BY 16"" PLATES, ROOF MATS 2 CRIBS BUILT IN INTERSECTION. AREA DANGERED OFF AND CRIBBED.
AN EXTENSION OF A FALL REPORTED 1-19-00 WAS DISCOVERED 4-25 ON A-4 MAINS. THE FALL EXTENDED AN ADDITIONAL 160 FT DOWN NO. 3 ENTRY THRU THE INTERSECTION AT SURVEY STATION NO. 1004 THRU THE 3-4 CROSSCUT AND NO. 4 INTERSECTION. THE TOTAL FALL EXTENDED APPROX 240 FT LONG BY 18 FT WIDE BY 7-9FT HIGH. THE FALL HAD BEEN DANGERED OFF.
DURING AN EXAMINATION OF AN UNINTENTIONAL ROOF FALL ANOTHER FALL WAS DISCOVERED AT SPAD 1469 MEASURING 8 FT THICK 20 FT WIDE APPROX 50 FT LONG. CRIBS AND TIMBERS WERE SET TO DANGER AREA OFF AN D FOR AN ADDITIONAL SUPPORT DOWN #2 RETURN.

ROOF FALL ON THE 2ND MAIN NORTH BELT JUST INBY THE 123 X CUT INTERSECTION WHICH INTERSECTION WAS OVERCAST.T HE FALL IS 20 FT L, FROM 14 TO 2FT WIDE AND 7 TO 8 FT HIGH AND WAS DUE TO DETERIORAT ION OF THE IMMEDIATE ROOF. THE AREA WAS DEVELOPED 6/92 WITH CONTINUOUS MINER AND SUPPORTED WITH 8 FT POINT ANCHOR ROOF BOLTS WITH HEADER BOARDS AND/OR 2"" PLANK.
A ROOF FALL WAS DISCOVERED ON D MAINS NO. 7 ENTRY STARTING 40 FT IN BY SURVEY STATION NO 25932 AND CONTINUING 170 FT THRU 2-4 WAY INTERSECTION. THE FALL WIDTH WAS APPROX 18 FT WIDE AND THE HEI GHT WAS APPROX 6-8 FT. THE FALL CONTAINED 5 FT FULLY GROUTED RESIN AND TENSION BOLTS. FALL WAS DANGERED OFF AND FLOOR TO ROOF SUPPORT ADDED AT APPROACHES. FALL WILL NOT BE CLEANED UP.
A FALL WAS DISCOVERED 4-23 AN A-4 MAINS NO. 26 BREAK NO.1 ENTRY INTERSECTION TO NO.4 ENTRY AT SURVEY STATIONS 1519, 1496 AND 1495. THE FALL WAS APPROX 190 FT LONG BY 18 FT WIDE BY 7-9 FT HIGH. THE FALL CONTAINED 6 FT RESIN BOLTS AND CRIBS. THE FALL WILL NOT BE CLEANED UP.
WHILE MAKING WEEKLY EXAM-FOUND A ROOF FALL IN OLD WORKS-ONF-1 MAINS PANEL IN OLD WINCHESTER MINE, #3 ENTRY !BT OUTBY SPAD #422. FALL IS 25'X25'X6' THICK. AREA WAS WORKED IN 1996 AND HAS BEEN A BANDONED EXCEPT FOR BLEEDER PURPOSES-THIS IS NOTTRAVELWAY OR WALKWAY AND FALL WAS DANGERED OFF AND WILL NOT BE CLEANED UP.
THE MIRACLE RUN PASSENGER ELEVATOR WAS DESCENDING THE SHAFT WHEN IT SUDDENLY STOPPED. ONE PASSENGER IN ELEVATOR OUT BY 11:10PM.
A FALL OF THE ROOF OCCURRED IN THE SUPPLY SLOPE APPROX 11080'-1200' APPROX L 8', W 12', T 6'. THE AREA WAS BOLTED WITH 48"" FULLY GROUTED ROOF BOLTS.

A ROOF FALL WAS DISCOVERED ON D-8 PANEL NO. 2 ENTRY 4-WAY INTERSECTION, 1 BREAK IN BY SPAD 6137 IN THE LAST OPEN BREAK. A ROOF BOLT MACHINE WAS PARKED IN THE EDGE OF THE INTERSECTION AND THE C ABLE WAS UNDER THE FALL, THE CABLE WAS CUT AND THE MACHINE PULLED FROM UNDER THE EDGE OF THE FALL. THE FALL WAS 20 FT WIDE BY 20 FT LONG BY 16 FT HIGH AND WAS DISCOVERED 4-11-01.
A ROOF FALL OCCURRED ON NO.5 SECTION G PANEL H/GENTRY DURING DEVELOPMENT MINING AT SPAD 14530. IT MEASURED 57'L X 63'W X 25'H. ~5' OF THE INBY SITE FELL ONTO THE CONTINUOUS MINER PARKED INBY T HIS INTERSECTION. THE MINER WAS PARKED HERE WHILE THE SECTION WAS DOWN FOR PINNING THE FACES. NOPERSONS WERE INJURED.
2 CLAY VEINS RUNNING DIAGONAL IN NO.2 HEADING FOR ABOUT 60 FEET DROPPED OUT. STOPPED AT INTERSECTION WHERE 6 FEET BOLTS WERE USED.
AN UNPLANNED EXPLOSION OCCURRED IN THE COAL MILLDUCT WORK. TWO RELIEF VALVES WERE BLOWN, NO OTHER DAMAGE RESULTED. NO INJURIES OCCURRED.
SKIP DUMP ORE BIN WAS PLUGGED & A SKIP OF ROCK WAS LOADED. THE SKIP COULD HAVE BEEN DUMPED AT ANY TIME BUT WE DID NOT WANT TO ADD TO THE PLUGGEDBIN. THE EMERGENCY HOIST WAS USED TO LOWER & RAI SE EMPLOYEES INTO AND OUT OF THE MINE. WHILE THEHOIST WAS BRINGING PEOPLE OUT A SEAL LEAKED & THE LOW ENGINE OIL SWITCH SHUT THE MOTOR OFF. WE ADDED OIL TO GET THE HOIST TO THE SURFACE.
AN UNINTENTIONAL ROOF FALL HAS OCCURRED ON THE #2 UNIT IN THE FACE OF #5 AND EXTENDING THRU THE RT CROSS CUT OVER TO THE #6 AND EXTENDING INTO #6 RT CROSSCUT. THE AREA WAS BOLTED WITH 5' RESIN BOLTS. THE FALL MEASURES 120 FT X 20 FT X 7 FT HIGH. THERE WERE NO INJURIES OR PROPERTY DAMAGE REPORTED.
CAGE HOIST TRIPPED OUT. A FIELD EXCITER TRANSFORMER HAD A BLOWN WINDING WHICH CAUSED A FUSE TO BLOW IN FIELD EXCITER PACKAGE CIRCUIT. A REPLACEMENT TRANSFORMER WAS LOCATED, SHIPPED AND INSTALL ED ON WED APRIL 25, 01. THE CAGE WAS BACK IN SERVICE AT APPROX 10 PM.
TWO UNINTENTIONAL ROOF FALLS HAVE OCCURRED IN THE FIRST LEFT PANEL OF NORTH MAINS IN THE NO. 3 ENTRY X-CUTS INBY SURVEY STATION 56. BOTH FALLS MEASURING APPROX. 5' X 19'W X 40' LONG. THIS WAS FOUND DURING WEEKLY EXAM BY CO. EMPLOYEE & MSHA EMPLOYEE.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #5 ENTRY APPROX. 20,000 FROM ACTIVE FACE. THE FALL MEASURED 20 FEET. LENGTH 20 FT WIDTH HEIGHT 4 FEET. THE AREA WILL BE REHABILITATED.
ROOF FALL @ XCUT #6 INTERSECTION 1 TAILGATE ENTRY #1 ( FALL DISCOVERED DURING WEEKLY EXAM). FALL MEASURED 9' HIGH X 16' WIDE X 20' LONG. 72"" RESIN & 10' CABLE BOLTS WERE USED IN AREA. 10' CABL E BOLTS BROKE WITH ANCHOR POPINT STILL INTACT. SEPARATION OF ROOF STRATA DUE TO EXPOSURE OF AIR & WATER OVER TIME, I FEEL IS RESPONSIBLE FOR FALL AT THIS LOCATION.
A FALL WAS DISCOVERED WHEN TRACKING THE SOURCE OF A BROKEN WATER LINE. THIS ENTRY IS NEVER TRAVELED WHEN MAKING EXAMS. THE ROOF WAS SUPPORTED WITH 6' TENSION REBAR BOLTS. THE FALL OCCURED IN # 7 ENTRY N.W. MAINS 20' OUT BY SS# 2511. FALL MEASURED 110' BY 20' BY 10' THICK. A 6"" PVC WATER LINE WAS INVOLVED IN THE FALL. THE VENTILATION WAS NOT DISTURBED. WILL NOT BE CLEANED UP.
THE WITH OF THE ADVANCING 10 ENTRIES COMBINED WITH THE FRAGILE ROOF CONDITION MAY HAVE CONTRIBUTED TO THE FALL, IN ADDITION TO A SOFT LAMINATED LAYER OF MATERIAL AT 4 FT.
A ROOF FALL WAS DISCOVERED ON A-MAINS, NO. 8 ENTRY & INTERSECTION AT SURVEY STATION NO. 800. THEFALL WAS APPROX. 60' LONG X 30' WIDE X 8-10' HIGH. THE FALL CONTAINED 6' RESIN BOLTS & WAS DEVEL OPED JAN 1998. THE FALL WAS DANGERED OFF AND TIMBERS SET AT APPROACHES. IT WILL NOT BE CLEANED UP AT THIS TIME.
DURRING THE WEEKLY EXAMINATION OF THE INTAKE ESCAPEWAY, A ROOF FALL WAS FOUND IN THE INTERSECTION OF THE 5TH X-CUT UG IN THE #7 INTAKE AIR ENTRY. APPROX. 20'X 30'X 5'HIGH. 4FT.FULL GROUT RESIN RODS WERE USED IN THIS AREA AND RIBS HAD BEEN NOTICED. THE FALL WAS TIMBERED OFF ON ALL SIDES AND TRAVELWAY WAS RE-ROUTED AROUND FALL.
AN UNINTENTIONAL ROOF FALL OCCURRED ON 1 SOUTH BELT AT #27 WALL. THIS WAS AN OLD FALL AREA THAT FELL BEFORE. THE FALL MEASURED APPROX 17' WIDEBY25' LONG BY 20' HIGH. NO EQUIPMENT DAMAGE OR INJ URIES.
A ROOF FALL MEASURING 60' LONG, 19' WIDE, AND 6' HIGH HAD OCCURRED IN THE #8 ENTRY RETURN OF THE SOUTHWEST SUBMAIN AT SPAD 25 + 50, APPROX 540' OUTBY TAILPIECE. DUE TO HIDDEN SLIP. THE AREA HA D BEEN TIMBERED AND BOLTED. THE FALL HAD BEEN TIMBERED AROUND.
THE MINE HAD AN UNINTENTIONAL FALL ON THE #11 BELT APPROX. 40'L X 20'W X 3' H.
A ROOF FALL HAS OCCURRED IN THE #1 ENTRY OF THE RETURN. THE SPAD LOCATION IS #133. IT FELLIN AN INTERSECTION. APPROX MEASUREMENTS ARE 14' L X 20' W X 6' H. THE AIR FLOW IS NOT BLOCKED OFF.
A ROOF FALL MEASURING 20' WIDE BY 70' LONG BY 8' THICK WAS FOUND BY MINE PERSONNEL AT APPROX 2:30 AM ON THE 010 MMU. THE FALL IS LOCATED BETWEEN THE NO. 22 AND 24 IN THE #1 ENTRY.
HOISTMAN STARTED AT 5:50 A.M. TO TEST #31 HOIST AND COULD ONLY LOWER HOIST. WHEN HE TRIED TO RESET POWER IT WOULDN'T RESET. MSHA WAS CALLED AT 6:25 A.M. AND TOLD THE HOIST WAS DOWN. WE DIDNT H AVE ANYONE UNDERFOND AT THE TIME. THE ELECTRICIANS FOUND A FAULT IN THE UPPER HOIST LIMIT SWITCH AND REPAIRED IT. THE HOIST WAS RETURNED TO SERVICE AT 7:05 AM SAME DAY AND MSHA WAS CALLED AND
ROOF FALL ON #4 ENTRY BETWEEN 4+20 & 4+80 ON SOUTH PANEL BELT. MMU-201.
HOIST WAS SURGING. ELECTRICIANS WERE CALLED AND THEY FOUND TWO RIBBON CABLES IN THE DC DRIVE UNIT THAT NEEDED TO BE REPLACED. THE HOIST WAS DOWN FOR APPROX. 1 HOUR FOR THE REPAIR. 11:30 AM TO 12:30 PM. THE REPAIR WAS MADE IN THE MCC ROOM ATTHE HOIST HOUSE.

DOZER OPERATOR WAS PUSHING DIRT INTO VALLEY FILL WHEN HE SAW A MIST FLOWING OVER DOZER BLADE. HE SLOWED THE THROTTLE BACK AND AS HE REACHED FOR PARK BRAKE HE SAW FLAMES IN FRONT OF LEFT DOOR O N THE OUTSIDE.

ALL PERSONNEL EXCEPT THOSE REQUIRED TO CORR ECT THE SITUATION WERE EVACUATED FROM THE MINE. EMPLOYEE WAS APPARENTLY INSPECTING CONVEYOR BELT TAKE-UP AT 2ND M.NORTH DRIVE WHEN HIS LEFT ARM BECAME ENTANGLED BETWEEN THE BOTTOM BELT AND THE STATIONARY ROLLER, CAUSING AVULSION LEFT ARM AN D FACIAL INJURIES. ACCIDENT IS CURRENTLY UNDER INVESTIGATION BY COMPANY, STATE AND MSHA OFFICIALS. AUTOPSY PENDING.

THE VICTIM WAS NOT INVOLVED IN THE INSTALLATION OF THE BELTS, BUT W ANTED TO FIX THE TWIST. AT THIS TIME THE MOTORMAN SET THE BREAKER ON THE MOTOR. THE VICTIM REACHED FOR THE BELT AS THE BREAKER WAS SET. COMPRESSOR STARTED HAND WAS CAUGHT BETWEEN BELT & PULLE
THE EE WAS USING A JACKLEG DRILL FROM A BOLT DECHE INSERTED A STEEL & BEGAN TO DRILL THE JACKLEGKICKED BACK HE PINCHED HIS RIGHT LITTE FINGER BETWEEN THE DECK RAIL & THE LEG OF THE DRILL. TH E FINGER WAS FRACTURED & THE EE WAS PLACED ON RESTRICTED DUTY. AMPUTATED 5/18/01
EMPLOYEE WAS WORKING AT ADVANCING POWER CABLES ALONG THE BELTLINE DURING A LONGWALL POWER MOVE. A LOOP OF 995 CABLE WAS ON THE BELT. ANOTHER EMPLOYEE FLIPPED THE LOOP OVER TOWARD THE INJURED EMPLOYEE. THE CABLE CAUGHT THE EMPLYEE'S RIGHT RING FINGER AGAINST THE BELT STRUCTURE.

HE WAS WORKING AROUND THE STEERING WHEEL, BENT OVER THE WHEEL TO TAKE THE PANEL OUT OF THE DASH AND FELT PAIN IN THE LOWER BACK AREA. HE WAS IN A BAD POSITION AS HE BENT OVER THE STEERING WHEEL. INJURY DIAGNOSED AS AN ACUTE STRAIN. HE IS EXPECTED TO MISS ABOUT ONE WEEK.

EMPLOYEE WAS TURNING WHILE THROWING A SMALL PIECE OF STONE AND FELT A ""POP"" IN HIS BACK, HE THEN BEGAN TO FEEL PAIN IN HIS BACK.
THE EMPLOYEE WAS REMOVING COVERS FROM THE HEAD DRIVE, ONE OF THE COVERS SLID AND CAUGHT HIS LEFT FOOT BETWEEN THE COVER AND THE HEAD DRIVE. EMPLOYEE WORKED UNTIL 5-17-01 WHEN AN X-RAY REVEALED A CHIPPED BONE ON HIS ANKLE. A CAST WAS APPLIED AT THAT TIME.
EE WAS PULLING MINER CABLE OUT OF ROADWAY AND FELT A PAIN IN LOWER RIGHT SIDE OF BACK. HE FINISHED HIS SHIFT AND WENT TO DR. HAS A CUTE L/S SPRAIN. ADVISED TO TAKE A FEW DAYS OFF IF NO LIGHT D UTY WAS AVAILABLE.
EE WAS STANDING ON OPERATOR'S SIDE OF ROOF BOLTER CHANGING A BIT ON HIS DRILL STEEL. SLATE FELL FROM BETWEEN BOLTS, STRIKING HIM ON THE L SHOULDER. EE CONTINUED WORK UNTIL 5:55 PM WHEN HE LEFT WORK TO HAVE IT CHECKED.
EMPLOYEE WAS LAYING DOWN PUTTING A GREASE PLUG IN THE SECTION BELTTAIL ROLLER WHEN A PIECE OF ROCK MEASURING APPROX 24 INCHES BY 30 INCHES, 3-4INCHES THICK FELL AND HIT HIM ON THE RIGHT LEG.
INDIVIDUAL WAS CLEANING UP BOXES AND PALLETS AND STEPPED ON A ROCK AND TURNED HIS ANKLE AND KNEE.

EE TRIED TO HOLD ROLLED UP BUTTON BOX ELECTRICAL CORD WITH HIS ARM. TOO MUCH WEIGHT CAUSING SPRAIN TO LEFT ARM.
EMPLOYEE WAS LIFTING HIS TOOL BOX FROM THE BACK OF THE SERVICE TRUCK. HE FELT SOMETHING PULL IN HIS GROIN. HE DID NOT GO TO THE DOCTOR OR MISS ANY WORK UNTIL 4-30-01, WHEN HE SAW THE DOCTOR HE WAS TOLD THAT HE HAD A HERNIA AND NEEDED SURGERY. HE WORKED UNTIL 5-11-01 AND WAS SCHEDULED FORSURGERY ON 5-14-01. WE WERE NOT NOTIFIED UNTIL LATER THAT THIS OCCURRED.
EE WAS TRYING TO REMOVE A PUMP FROM A POND WITH ANOTHER EE OPERATING A BACKHOE. THE BACKHOE OPERATOR HAD THE MACHINE IDLED UP AND COULD NOT PICK UP THE PUMP. INJURED MAN WAS NEAR THE BUCKET OF THE BACKHOE WHEN THE OPERATOR BUMPED A LEVER AND THE PUMP HIT INJURED MAN'S HAND SMASHING IT BETWEEN THE PUMP AND BACKHOE BUCKET.

EE WAS OPERATING A 15 TON GREENBURG MOTOR, ON A TRIP REMOVING EQUIPMENT FROM THE MINE, CABLE HUNG ACROSS THE TRACK CAUGHT A POWER BOX ON THE LAST CAR OF THE TRIP, WHEN THE CABLE FLEW BACK IT K NOCKED HIM OFF THE MOTOR.

STEP OFF BACK OF COAL HAULER, HE STEP INTO HOLE AND TWISTED LEFT KNEE.

EE AND OTHER HAD LOADED BELT STRUCTURE ON A FLAT CAR AND WERE TRANSPORTING IT TO #1 SECTION THE STRUCTURE FOWLED AGAINST A LOW PLACE IN THE TOP. AND AS THEY WERE TRYING TO FREE IT UP. A PIECE OF TOP STRUCTURE SWUNG AROUND AND HIT EE INT HE LEG CAUSING THE INJURY.

EMPLOYEE WAS IN THE ENTRY OUT BY END OF A MANTRIP, TRAVELING TOWARD THE ELEVATOR AT THE END OF HIS SHIFT. EE LIT PILOT LIGHT TO DRYER BURNER AND HE TURNEDON GAS. THE PILOT WENT OUT AND EMPLOYEE WENT TO TURN OFF BLOWER WITHOUT TURNING GAS OFF. WHEN HE CAME BACK TO LIGHT PILOT, THE FIRE BACKFLASHED BURNING ONE HAND AND SIDE OF FACE.
EE WAS DESCENDING A LOADER LADDER WHEN HIS FOOT SLIPPED. AS HE CAUGHT HIMSELF TO KEEP FROM FALLING HE FELT A PULL IN THE GROIN AREA. THE EE WAS PUT ON MODIFIED DUTY, BUT A FOLLOW UP EXAM ON 5- 18-01 DIAGNOSIS WAS MADE FOR TEMPORARY DISABILITY.

EE WAS UN HOOKING PULL CHAIN WHEN SCOOP JUMPED INTO POINT, PINNING HIM BETWEEN TWO PIECES OF EQUIPMENT INJURING BOTH LEGS.

SUPERVISOR HAD TROUBLE WITH HIS JEEP SO HE WENT ON THE TROLLEY WIRE SIDE TO HIT CONDUCTOR BOX. HE REACHED UP TO TAKE TROLLEY POLE OFF THE WIRE & CONTACTED THE HARP. CAUSE OF INJURY AWARENESS.
EE WAS PUTTING SHEETS OF STEEL IN A RACK (PRYINGTWO 4X8 SHEETS APART). A SHEET OF METAL FELL TOWARDS HIM, SMASHING HIS FOOT & BREAKING TWO TOES.
EE BENT DOWN TO PICK UP A LOAD BINDER TO BIND A BUNDLE OF CRIBS TO A SHIELD TRAILER WHEN HE FELTPAIN IN HIS LOWER BACK AND DOWN HIS LEFT LEG.
EE IS UNCERTAIN BUT THINKS PAIN FIRST STARTED WHILE HE WAS PULLING MINER CABLE STRAINED AND SORE HIP AND LEG. WORKED UNTIL 4-23-01.

SUPERVISOR ADVISED HIM TO SEE DR. ASSOCIATE STATED HAD TRIPPLANNED FOR NEXT WEEK SOULD SEE DR UPON RETURN IF STILL IN PAIN RET'D 4/23/01 & PROCEEDED TO SEE DR WHO CONFIRMED HERNIA

EE C0OMPLAINED AT BEGINNING OF SHIFT OF PAIN IN HIS RIGHT KNEE. ABOUT 1100 HE TOLD THE FOREMAN THAT HIS KNEE FELT WORSE. AT THE END OF THE SHIFT HE DECIDED TO GO TO THE DOCTOR.
EE WAS LOADING TRUCKS WITH ROCK & WAS RETURNING TO PILE TO RELOAD BUCKET WHEN A TOOTH ON BUCKET CAUGHT A LARGE ROCK CAUSING THE LOADER TO STOP HARD. THIS IN RETURN JARRED HIS BACK.
EE WAS DESCENDING LADDER BY HYD SCALER. HIS FOOTSLIPPED OFF BOTTOM RUNG & HE HELD ON W/RIGHT HAND HE SAID HE FELT HIS RIGHT SHOULDER POP & SOME PAIN. HE REC'D TREATMENT CONSISTING OF PHY THERP & HOME EXERCISE W SOME RELIEF OFF & ON DID NOT MISS WORK DURING THIS PERIOD DR FINALLY RECOMMENDED SURGERY 10/31/01
EE ALLEGES HE STRAINED HIS BACK REMOVING A PIECE OF ROCK FROM THE BRIDGE.

REMOVING BROKEN CONTINUOUS MINER BITS WITH A PUNCH AND HAMMER, A PIECE OF THE PUNCH BROKE OFF & BECAME EMBEDDED IN HIS LEFT THIRD FINGER. THREE SUTURES WERE NECESSARY AFTER METAL WAS REMOVED.
EE CUT THE END OF HIS MIDDLE FINGER ON LEFT HAND OFF, WHILE HE WAS OPERATING BOLT MACHINE. HE HAD HIS HAND ON THE BOOM ARM, AS HE DROPPED HIS OF MACHINE.
STEPPING DOWN OUT OF TRUCK LEFT FOOT SLIPPED FELL AND ALL OF BODY WEIGHT WENT ON LEFT SIDE OF BACK.
INJURED STATES HE WAS DRILLING A HOLE FOR A ROOFBOLT & A PIECE OF ROCK FELL & HIT INJURED ACROSSTHE BRIDGE OF NOSE. WHEN HE MOVED BACKWARDS ANOTHER PIECE FELL AND HIT LEFT SHOULDER. INJURED WA S TAKEN TO HOSPITAL AND RECEIVED 16-17 STITCHES.

EE STATED: HELPING AT NEW LONGWALL FACE SET-UP. WHEN LIFTING AN EMULSION HOSE UP TO THE HANGER, EXPERIENCED A POSSIBLE STRAIN TO HIS SHOULDER.
EE WAS USING A HAND SHOVEL TO CLEAR A FEED HOPPER AND INJURED HIS LOWER BACK, AGGREVATING A PREVIOUS INJURY.
EE WAS PULLING SHIELDS AND AS HE TURNED, CAUGHT HIS RT FOOT AND FELL CAUSING HIM TO TWIST HIS BACK.

EE WAS USING A SCOOP TO REMOVE A BRATTICE & HIT COAL RIB WITH SCOOP STRIKING BACK AGAINST SCOOP SEAT CAUSING A BACK STRAIN.
EE WAS IN THE DECK OF HIS SCOOP WHEN A PIECE OF LOCK APPROX 3'X2'X3"" FELL BETWEEN ROOF BOLTS STRIKING HIM IN THE UPPER BODY.
THE EE WAS CHIPPING HARDENED CEMENT OUT OF THE CEMENT SILO DISCHARGE CONE. HE AND SEVERAL OTHER EES HAD WORKED ON THE PROJECT. THEY WERE WEARING PPE-GLASSES AND GOGGLES. THE EE COMPLAINED OF E YE PAIN AND HAD HIS EYES FLUSHED ON SIT AND AT ADR.'S OFFICE. ON 4-9-01 HE SAW AN OPHTHAMOLOGIS WHO PRESCRIBED TIME OFF WORK TO LET HIS EYE HEAL. SCRATCH DETECTED.
EE WAS CLIMBING DOWN A FOUR FOOT LADDER OUT OF A GENERATOR TRAILER AND STEPPED TO THE GROUND TWISTING HIS LEFT ANKLE.
EE WAS CHANGING A VALVE CHEST INSIDE THE CONFINES OF A SHIELD. WHEN HE CAME OUT AND TRIED TO STAND UP HIS LEFT HIP AND LEG WAS NUMB. CAUSE OF INJURY WAS AWKWARD POSITIONING. THE EE WAS WALKING ACROSS THE SHOP FLOOR WHEN ANOTHER MECHANIC ASKED HIM A QUESTION. AS HE CONTINUED WALKING, HE TURNED TO ANSWER AND AS HE DID, A SHARP PAIN IN HIS LOWER BACK CAUSED HIM TO ST OP WALKING & BECOME IMMOBILE. DIAGNOSED AS A SEVERE MUSCLE STRAIN/SPASM IN HIS LOWER BACK.
EE WAS BENT OVER ATTEMPTING TO UNCOUPLE SUPPLY CARS AND EXPERIENCED LOW BACK PAIN. DIAGNOSED AS HAVING A PROGRESSION OF A DEGENERATIVE CONDITION(THIS REPORT IS BEING SUBMITTED TO ABATE CITATIO N #4718262. WE DO NOT BELIEVE THIS TO IS A REPORTABLE INJURY AND THE CITATION WILL BE CONTESTED)
EE AND ANOTHER EE WERE HANGING A HIGH VOLTAGE CABLE WHILE HE WAS LIFTING HE FELT A STRAIN IN HIS SHOULDER AREA.
EE WAS BACKING UP TO THE DUMP AREA AND WENT THROUGH A BERM. THE TRUCK TRAVELED ABOUT 25' THEN FLIPPED ON IT'S TOP. TOTAL DISTANCE TRAVELED WAS APPROX. 50'. THE SEAT BELT BROKE AND EE LANDED ON THE CEILING.
GARDNER WAS CLIMBING THE STEPS TO GO INTO THE CAB OF THE D9R DOZER THAT HE WAS GOING TO OPERATE.WHILE DOING THIS HE SLIPPED WHICH CAUSED A LOT OF HIS BODY WEIGHT TO BE SHIFTED BY HIS RIGHT AR M. THIS RESULTED INTO A STRAIN TO HIS RIGHT ELBOW. THIS HAS NOW BEEN DIAGNOSED AS A MUSCLE TEAR AND HE RECENTLY BEGAN TO MISS WORK DUE TO THIS PROBLEM.
THE MAN WAS WALKING DOWN ENTRY TO GET PLASTER WHEN HE STEPPED IN A HOLE AND TWISTED ANKLE.
EE WAS HELPING SET A TRASH PUMP. FELT PAIN IN LOWER BACK AND DOWN LEG.
EE WAS OPERATING GRADER DOWN HILL. THE ENGINE SHUT OFF, GRADER PICKED UP SPEED. HE PANICKED AND JUMPED FROM MACHINE. HE DID NOT TRY TO USE THE PARKING BRAKE. CAUSE OF INJURY WAS FAILURE TO USE BACK UP BRAKE. COMPLIANCE WITH RULE/REGS WAS FACTOR, PROTECTIVE EQUIPMENT WAS A FACTOR, MINER PROFICIENCY A FACTOR AND MINING EQUIP/SYSTEMS A FACTOR.

THE EQUIPMENT ROLLED BACKWARDS ABOUT 60-75' STRIKING THE WALL. THE SUDDEN STOP GAVE THE EE WHIPLASH. DOCTORS RELEASED HIM TO FULL DUTY. HOWEVER HE STATES HE IS HAVING HEADACHES & CANNOT RETURN TO WORK.
EE CLAIMS THAT WHILE LIFTING ROTARY KILN #2 WITH NATURAL GAS, THE FLAME BLEW BACK THROUGH THE HOOD WINDOW AND UNDER HOOD DOOR. THE FLAME BURNED EE'S LEFT FOOT. EE DECLINED TO SEE A DR UNTIL 4- 11-01. HE WAS RESTRICTED FROM RETURNING TO DUTY UNTIL THE DR SAW HIM AGAIN.
THE EMPLOYEE STATED THAT HE WAS WALKING ACROSS THE FACE WHEN HE STEPPED IN A HOLE AND TWISTED HIS RIGHT KNEE.
AS EE WAS CLIMBING THE BOARDING LADDER HIS TOTE BAG GOT CAUGHT ON THE TRUCK'S EMERGENCY SHUTDOWN SWITCH, AS HE WAS DESCENDING TO RELIEVE THE TOTE BAG, HE MISSED THE LADDER STEP AND FELL TO THE GROUND ON HIS RIGHT HEEL.
EE ALLEGES HE RECEIVED AN ELECTRICAL SHOCK FROM A DAMAGED ELECTRICAL CONDUCTOR ON THE CONTINUOUSMINER.
EE WAS WORKING OFF LADDER. HAD BUCKET OF SEALANTHANING ON SCREW DRIVER. REACHED TO LIFT BUCKET &FELT PAIN IN LOWER BACK.

EE WAS WALKING DOWN STEPS NEAR 33 BELT WHEN HE STEPPED ON A LUMP OF COAL TWISTING HIS RIGHT ANKLE.
WHILE TRYING TO REMOVE A STUCK PIECE OF BOTTOM ROCK FROM THE CRUSHER, THE ROLLER THAT THE EMPLOYEE WAS STANDING ON TURNED CAUSING HIS KNEE TO BE TWISTED.
WHILE DUMPING DIRT AT THE KRULOCK PIT NEAR CRESCENT OHIO A ROCK TRUCK DRIVEN BY KELLY FULTONG WENT OVER AN EMBANKMENT BACKWARDS. THE OPERATOR JUMPED FROM THE CAB INJURING THE HEELO ON HIS RIGH T FOOT. THE TRUCK LANDED ON ITS RIGHT SIDE AT THE BOTTOM OF THE EMBANKMENT RECEIVING MINOR DAMAGE.
