WE HAD REPEATED POWER FAILURE DUE TO INCOMING VOLTAGE LOW. THIS CAUSED THE DYNA-BRAKE TO LOOSE ITS MEMORY. RE-PROGRAMED DYNA-BRAKE. MSHA WAS NOTIFIED AT 12:15 PM THAT THE HOIST WAS DOWN AND WA S CALLED AT 1:00 PM WHEN BACK IN SERVICE.
A ROOF FALL 60' X 20' X 6' IN THE EAST PARALLELS #6 ENTRY, #3 XCUT. THE AREA WAS MINED IN MAY 2000 & SUPPORTED WITH 6' TENSION REBAR BOLTS & TIMBERS. THE FALL WAS CAUSED BY AN UNDETECTED SLIP IN THE MINE ROOF.
A FALL OF ROOF OCCURRED IN THE WEST MAIN ENTRIESIN THE #0 ENTRY BETWEEN X-CUT 23 AND 24, APPROX 12100' OUT BY THE UNIT. THE FALL WAS APPROX 35' LONG BY 18' WIDE BY 6 1/2' HIGH AND THE AREA WAS SUPPORTED WITH 6 FT FULLY GROUTED BOLTS. APPARENT CAUSE WAS AN UNDETECTED SLIP WITH WATER.
AN UNINTENTIONAL FALL OCCURRED AT BELT 39 OF THE #2 ENTRY 0OF THE 5 SOUTH LEFT RETURN. THE FALL WAS FOUND ON WEEKLY EXAMS. THE FALL MEASURED 20' X 20' X 5' AND WAS BOLTED WITH R' RESIN BOLTS. THERE NO INJURIES OR PROERTY DAMAGE.

A ROOF FALL WAS FOUND ON 5-30-01 AT XC 169 ENTRY#1. THE FALL WAS IN A FOUR WAY INTERSECTION THAT HAD BEEN MINED JAN 1998. THE FALL MEASURED 20' BY 30' LONG BY 10' HIGH. AREA HAD BEEN SUPPORTED WITH 8' FULL RESIN BOLTS. NO INJURY OR PROPERTY DAMAGE RESULTED.
A ROOF FALL WAS DISCOVERED ON C3 PANEL NO. 1 ENTRY, 1 BREAK IN BY SPAD 5753. THE FALL WAS APPROX 200 FT LONG BY 18 FT WIDE BY 10-12 FT HIGH. THEFALL CONTAINED 6 FT RESIN BOLTS AND THE AREA WAS DEVELOPED 2-28-01. THE APPROACHES WERE TIMBERED AND DANGERED OFF.
LAB TECHNICIAN DECIDED TO POUR A GLYCERIN ETHONOL AMMONIUM SOLUTION IN A CLINKER DRAG THAT TRANSPORTS CLINKER FROM CLINKER COOLER TO STORAGE. THE MATERIAL COMBINED WITH THE TEMP AND RED HOT CL INKER TO CAUSE AN EXPLOSION IN THE CLINKER ELEVATORS.
ROOF FALL OCCURRED IN THE P2SOUTH #6 ENTRY #19 CROSS CUT THE ROOF FALL WAS 19'WIDE, 19'LONG, 5 HIGH, LOCATED IN THE PRIMARY AIR COURSE AT A FOUR WAY INTERSECTION. THE ROOF FALL WAS TIMBERED O FF AND THE PRIMARY ESCAPEWAY WAS REROUTED. THE FALL WILL NOT BE CLEANED UP.

FALL WAS 20' WIDE 30' LONG BY 14'HIGH IN A 4 WAY INTERSECTION THAT WAS MINED APRIL 2001 AND SUPPORTED WITH 6 AND 8' FULL RESIN BOLTS. NO INJURY OR PROPERTY DAMAGE RESULTED. THERE ARE NO PLANS TO CLEAN UP THIS FALL.

THE FALL WAS APPROX 7100' FROM AN ACTIVE FACE. THE FALL WAS BREAKERED OFF FROM ALL ENTRANCES.FALL WAS DISCOVERED AT APPROX 3:30 PM ON 5-29-01.
A ROOF FALL OCCURRED ON THE 002 SECTION AT #70 CROSSCUT IN THE GOB AND PILLAR CUT AND RODE OUT BY THE MOBILE ROOF SUPPORTS APPROX 25'. THE FALL WAS APPROX 25' LONG BY 35' WIDE BY 8' THICK. 4' BOLTS HAD BEEN USED.
AN UNPLANNED FALL OF ROOF OCCURRED ACROSS A LINE OF CROSSCUTS. THE ROOF WAS SUPPORTED ON THE ADVANCE WITH 4' RESIN RODS. THE FALL WENT ACROSS ALL ENTRIES AND WAS 5 1/2'-6' THICK. THE FALL WAS DISCOVERED DURING A WEEKLY EXAMINATION.
AT APPROX 11:00 PM THE FIRE DEPT WAS CALLED TO EXTINGUISH A LARGE CONVEYER BELT FIRE AT #8 BELT SOUTH OF THE RIVER. THE BELT FIRE IS BEING INVESTIGATED BY THE FIRE DEPT AND ILLINOIS FIRE MARSH AL AS AN ARSON BECAUSE A GAS CAN WAS FOUND AT THE SCENE AND THERE WAS NO OTHER FIRE SOURCE.

ON SUN. MAY 27, 2001, AT APPROX.7:15AM, THERE WAS AN EXPLOSION ON THE NO. 3 BOILER IN THE POWER HOUSE, NO INJURIES. IT WAS INITIALLY BELIEVED THAT THE RUPTURE WAS CAUSED BY STEAM. THE ONLY DEF INITION OF AN ACCIDENT WAS ""AN UNPLANNED IGNITION OR EXPLOSION OF GAS OR DUST"". INVESTIGATION IS ONGOING AND NO DEFINITE CONCLUSIONS HAVE BEEN DRAWN TO DATE.

A FALL OF ROOF OCCURRED IN THE 1ST SOUTH WEST SUBMAINS IN THE #1 ENTRY AT X-CUT #10. THE FALL WAS APPROX 6 1/2' HIGH 54' LONG 19' WIDE AND THE APPARENT CAUSE WAS AN UNDETECTED SLIP. THE AREA WAS SUPPORTED WITH 6' FULLY GROUTED BOLTS AND 2 PROP SETTERS.
THE CONTINUOUS MINER WAS CAUGHT IN A PILLAR LINEFALL, THE FALL ORIGINATED TO THE RIGHT OF THE PILLAR BLOCK BEING MINED AND FELL ACROSS THE TOP OF THE PILLAR LINE TOWARD THE BLOCK THE MINER WA S WORKING IN. NO EQUIP DAMAGE, NO BLOCKAGE OF VENTILATION OR ESCAPEWAY. NO INJURIES, WILL NOT BE CLEANED UP, BREAKERED AND DANGERED OFF.

A PILLAR BOUNCE OCCURRED IN THE 14 NORTH DEVELOPMENT SECTION MMU 002-0 BETWEEN 15 AND 16 X-CUT IN THE #2 ENTRY. FOR 90 FT, ONE TO 2 FEET OF RIB LINE BLEW OUT ON BOTH SIDES OF THE ROADWAY, 2 C ONTRACT MINERS WERE INJURED WHILE HINGING MINER CABLE ON THE RIGHT SIDE OF ROADWAY.
A ROOF FALL MEASURING 20' L X 20'W X 4'H, OCCURRED IN THE #4 ENTRY OF 1ST LEFT SECTION AT SURVEY STATION 397. THE FALL WHICH IS IN AN INACTIVE AREA OF THE MINE WAS DISCOVERED BY THE MINE EXAMI ONER ON HIS WEEKLY RUN. THE FALL DOES NOT AFFECT TRAVEL OR VENTILATION AND WILL NOT BE CLEANED UP.

THE ELEVATOR MALFUNCTIONED IN THAT THE DOOR WILL NOT OPEN WHEN DROPPED TO BOTTOM OF SHAFT. ELECTRIC CONTROL PANEL FAILED.
ROCKFALL IN #6 ENTRY & #7 ENTRY INTERSECTION OF RIGHT RETURN. FALL WAS 4 FT THICK TO 8 FT THICK, 20 FT WIDE, 80 FEET LONG.
DURING WEEKLY EXAM OF AIR COURSE A FALL WAS OBSERVED IN THE NORTH EAST RETURN 15 X-CUT. DIMENSI ONS APPROXIMATELY 12' X 10' X 8'. NO INTERRUPTIONS IN VENTILATION OR INJURIES. THE AREA WAS POST ED OFF AND ADDITIONAL SUPPORTS INSTALLED IN THE INTERSECTION.
THE ELECTRICIAN HAD BROUGHT THE ROOM THROUGH THE INTERSECTION ABOUT 10 MINUTES BEFORE THE ROOF GAVE WAY. HE NOTICED THE ROOF LOOKED LIKE IT SAGED. THE ROCK STARTED BREAKING OUT BETWEEN THE BOL TS THEN ALL OF IT CAME IN. THE FALL WAS APPROX 16 FEET WIDE, 25 FEET LONG AND ABOUT 7 FEET HIGH. SUPPORTED W/ 6 FOOT TENSION REBAR. FALL OCCURED2 WEST SECTION, NO. 7 ENTRY, 2 BLOCKS INBY 4025

THE FALL MEASURED 4' H IGH X 30' LONG X 25' WIDE AND IS LOCATED APPROXIMATELY 4000' OUTBY THE 001 SECTION(""T"" SECTION) ON THE MAIN LINE. THE FALL WILL NOT BE CLEANED UP AND ALL APPROACHES TO THE FALL HAVE BEEN TIMB
SOME ROCK FELL ON THE RIPPER HEAD OF JOY CONTINUOUS MINER, THE ROCK THAT FELL WAS NOT ABOVE THE ANCHORAGE OF BOLTS AND THEREFORE MANAGEMENT FEELS THIS WAS NOT A REPORTABLE OCCURANCE. MANAG EMENT REPORTED IT, BUT WILL PROTEST IT.

WHILE HAULING OVERBURDEN MATERIAL, A FIRE BROKE OUT ON 240 TON HAUL TRUCK. AFTER THE TRUCK STOPPED, THE OPERATOR ENGAGED THE DUMP BRAKE, DISCHARGED THE FIRE SUPPRESSION SYSTEM AND DISMOUNTED T RUCK. THE OPERATOR RETIRED TO A SAFE LOCATION AND WAS UNINJURED. THE HAUL TRUCK IS A TOTAL LOSS. MSHA WAS NOTIFIED OF THE INCIDENT.
EVIDENCE WAS FOUND AT THE INDIAN CREEK AND 7J SHAFTS ON 5-18-01 THAT AN IGNITION HAD OCCURRED IN THE SEALED AREA OF 7 HAULAGE. NO INJURIES OCCURRED FROM THIS EVENT. SHAFT CAPS DISLODGED WITH M INIMAL DAMAGE.
A ROOF FALL WAS FOUND IN THE NO. 3 ENTRY AT SPADNO. 70 ON THE NORTH EAST MAIN SECTION WHILE MAK ING THE WEEKLY EXAM ACCORDING TO 75.364. THE ROOF FALL WAS 11 FT LONG, 9 FT WIDE AND 5.5 FT HIGH . THE AREA WAS DEVELOPED 2-10-01. THE FALL WAS DANGERED OFF, CRIBBED AND NOT CLEANED UP. THERE WERE NO INJURIES OR PROPERTY DAMAGE NOR WAS VENTILATION AFFECTED.

AFTER DRILLING A HOLE FOR WATER IN AN ABANDONED MINE (#6 HIGHWALL) AN IGNITION WHILE CUTTING A WALL CASING. THE PIPE WAS APPROX 11 FT ABOVE THE GROUND AND WAS 10 INCHES IN DIAMETER. WHILE CUTT ING THE PIPE, A PIECE OF HOT METAL ON A SPARK FELL IN THE HOLE CAUSING AN IGNITION. A GUSH OF AIR WAS REPORTED, NO FLAMES.

INJURED WAS PUTTING OIL IN THE CENTER GEAR CASE OF THE #11 JOY MINER ON #2 UNIT WHEN A PIECE OF ROCK FELL FROM THE ROOF STRIKING INJURED IN THE BACK OF THE HEAD AND SHOULDERS PINNING HIS HEAD ON THE MINER. INJURED RECEIVED SEVERE HEAD INJURIES. INJURED WAS LIFE FLIGHTED FROM THE MINE TO ST MARY'S HOSPITAL IN EVANSVILLE, IND. INJURED DIED ABOUT 6:15 PM ON MAY 23, 2001.
THE VICTIM WAS REPAIRING A SKID STEER LOADER WHEN THE HYDRAULICALLY-CONTROLLED LIFTING ARM WITH FORKS DROPPED, PINNING HIM TO GROUND
THE EE LEFT INDEX FINGER WAS STRUCK BY ROCK WHILE HE WAS OPERATING A JACKLEG DRILL. THE FINGER WAS FRACTURED AMPUTATION AT THE PROXIMAL JOINT MIDDLE & DISTAL SECTIONS WERE REMOVED
WHILE REINSTALLING REVERSE AIR FAN BEARING, SHAFT STARTED TO TURN. EE TRIED TO RE ATTACH PIPE WRENCH ON SHAFT & GOT CAUGHT TWISTING DOWN & CATCHING EE'S FINGER BETWEEN PIPE WRENCH HANDLE & BASE OF BEARING SUPPORT
EE HAD COMPLETED HANGING FLYPADS. AS HE WAS WALKING AWAY FROM FLYPADS, THE SHUTTLE CAR OPERATOR RANG BELL THREE TIMES AND THEN PROCEEDED THROUGH CURTAINS. THE SHUTTLE CAR HIT INJURED IN THE BA CK OF HIS LEGS AND CARRIED HIM AS IT HIT THE PARKED SCOOP. EE WAS TRANSPORTATED TO HOSPITAL. INJURY RESULTED IN BI-LATERAL AMPUTATION OF LEGS BELOW KNEES.
THE EMPLOYEE WAS GUIDING A BELL HOUSING DOWN OFF THE METCALF SECONDARY CRUSHER OVERHEAD CRANE WHEN A CLEVIS PIN FELL 45 FT., HITTING THE EMPLOYEE ON THE RIGHT HAND PINKY FINGER.
EE WAS HELPING A PHELPS DODGE EE HOOK UP THE CABLE ON THE ROCK GUARD OF ONE OF THE CRANES AT PRIMARY CRUSHER. THE HOOK DID NOT SETTLE UNDER THE CABLE AS THEY HAD HOPED. PHELPS DODGE EE BROUGHT HOOK UP A LITTLE. OUR EE SAID HIS FINGER WAS CAUGHT IN THE SHEAVE. PHELPS DODGE EE LOWERED THE HOOK WHEN HE REALIZED OUR EMPLOYEE'S FINGER WAS CAUGHT

EE WAS SECURING CAP AND CABLE TO DRILL STEEL INSIDE DERRICK OF T4 DRILL. APPARENTLY THE DRILL STEEL MOVED AND FELL ON OR AGAINST HIS LEFT INDEX FINGER, AMPUTATING THE TIP.
WHILE INSTALLING V-BELTS GOT FINGER CAUGHT BETWEEN V-BELT AND SHEEVE.
EMPLOYEE WAS REMOVING BACKING FROM HP300 CONE BOWL- THE BACK SLID DOWN BETWEEN FRAME AND MANTLE STRIKING RIGHT HAND. 2 BROKEN FINGERS AND LACERATIONS.

THE EE WAS OPERATING A BOBCAT CLEANING THE BOTTOM FLOOR OF PREP PLANT, HE RAN OVER A CURB JARRING HIS BACK.
DURING BREAK TIME HE WAS UP ON TOP OF A STONE. WALKED TO ONE END AND STONE TIPPED OVER, WITH HIM FALLING TO GROUND.
EE SLIPPED AND FELL, CAUSING INJURIES TO HIS RIGHT AND ELBOW. EE WORKED UNTIL SURGERY ON 8-13. PRE-EXISTING.
WHILE OPERATING WATER WAGON, HE HIT A BUMP. THE JOLT THREW HIM INTO THE SEAT BELT WITH ENOUGH FORCE TO CAUSE INJURY. THE BUMP WAS CAUSED WHEN AN OPERATOR CUT TOO DEEP WHEN PUCHING UP THE STOCK PILE. IT WAS APPROXIMATELY 14"" HIGH.
INJURED WAS PULLING THE CONTINUOUS MINER TRAILING CABLE BY HAND WHEN HE FELT A SHARP PAIN IN HISBACK.
TRAMMING PUSH-PULL MONORAIL WHEN BOLT BROKE CAUSING UNIT TO FALL DOWNWARD RAPIDLY, PUSHING ARM DOWN.
EMPLOYEE WAS SWEEPING THE SHOP FLLOR AROUND A DOZER BLADE THAT HAD BEEN REMOVED FROM THE DOZER. HE STEPPED OVER THE PUSH ARM OF THE DOZER BLADE AND TWISTED HIS KNEE. HE STEPPED ON DIRT CLODS I N THE FLOOR CAUSING THE INJURY.
THE EMPLOYEE STATED HE WAS WALKING THE RETURN AIRWAY IN 35 LEFT TO BEP 32 AND WAS WALKING A WOODEN BRIDGE AND THE CRIB BLOCKS WAS NOT ATTACHED TOO THE PIPES THEY WERE LAYING ON AND THE WOOD H EADER CAME OFF THE PIPE CAUSING THE EMPLOYEE TO FALL IN THE WATER AND A HOLE TWISTING HIS LEFT KNEE AND STRIKING HIS LEFT SHOULDER.
WHILE TRAVELING THROUGH MINE IN SHUTTLE CAR, EE ENCOUNTERED A LARGE HOLE IN THE MINE FLOOR, BUMPING HIM AGAINST THE SEAT OF THE SHUTTLE CAR CAUSING PAIN TO HIS LOWER BACK WITH PAIN MOVING TO H IS RIGHT SIDE. LOST TIME BEGAN 6/4/01.
EE WAS ASSISTING ANOTHER PERSON CARRY A STEEL ROLLER FOR A BELT CONVEYOR, FOOT BECAME ENTANGLED IN MUD, FELT PAIN IN LOWER BACK.
EE WAS IN THE PROCVESS OF WELDING A BRACKET FOR A FIRE HOSE WHIL STANDING ON THE 3RD STEP OF A 6' STEP LADDER. THE LADDER SLIPPED OUT FROM UNDER HIM AND HE FELL ON THE LADEER. THE FALL RESULTE D IN A FRACTURED RIB AND LOSS OF WORK.
EE WAS PULING ON A RIPPER MOTOR WHILE IN A BENT OVER POSITION.
EE REACHED DOWN TO PICK UP WASH DOWN HOSE AND STRAINED MIDDLE SECTION OF BACK, EE WAS PREPARING TO USE THE HIGH PRESSURE HOSE TO WASH OUT THE BUCKET OF THE CAT 966 LOADER HE WAS OPERATING. EE MISSED FIRST SCHEDULED WORK SHIFT, SAT., 6-2-01.
THE EMPLOYEE WAS INSTALLING ROOF BOLTS IN 1 AC ENTRY OF THE 5J SECTION. A COAL RIB ROLLED OUT STRIKING EMP ON THE LOWER LEFT LEG.
EMPLOYEE AND 1 MAN WERE CARRYING WOODEN PROPS AS A TEAM. THE MAN IN THE REAR STUMBLED WHICH CAUSED HIM TO STEP AWKWARDLY TO KEEP FROM FALLING. HE FELT PAIN IN HIS KNEES BUT HE FINISHED THAT SH IFT AND 2 MORE. THE PAIN GOT WORSE SO HE WENT TOTHE HOSPITAL AND HAS BEEN OFF WORK SINCE THEN.
MAINT FOREMAN WAS HELPING HIS THIRD SHIFT CREW DURING A #5 KILN SHUTDOWN BY HELPING REMOVE A BEAM OUT OF #5 COOLER. WHILE STANDING ON THE BURNING FLOOR AT APPROX 5:30 AM THE BEAM HIT THE SIDE OF THE COOLER AND DUST FROM ABOVE FELL BEHIND HIS SAFETY GLASSES INTO HIS RIGHT EYE CAUSING AN IRRITATION AND CORNEAL ABRASION. HE SOUGHT MEDICAL CARE LATER THAT DAY TO RECOVER.
EE WAS TRYING TO REMOVE ROCK THAT WAS WEDGED BETWEEN TWO TIRES. ROCK PUNCTURE THE TIRE EXPLODED THROWING METAL & RUBBER. EE WAS HIT IN FACE INJURY TO RIGHT EYE.
WHILE OPERATING JACK HAMMER, THE EMPLOYEE'S HANDSLIPPED AND WAS CUT BY WIRE COUPLING HOLDING THEAIR HOSE TO THE JACK HAMMER.
EMPLOYEE WAS PERFORMING MECHANICAL DUTIES AT BUCKET ELEVATOR #11. THE EMPLOYEE WAS HOLDING A WHEEL AS LONG AS A PARTNER WAS HITTING THE WHEEL WITH A SLEDE HAMMER. ACCIDENTLY THE SLEDGE HAMMER SLIPPED FROM THE WHEEL AND HIT EE'S HARD HAT CAUSING HIM A CONTISION AND A SMALL WOUND.
EE WAS HELPING TWO OTHERS MOVE MINER CABLE OUT OF ROADWAY AND AS HE PULLED ON THE CABLE, HE FELT PAIN IN HIS GROIN AREA. BEGAN MISSING WORK 6/8/01.

SMALL PIECE OF ROCK FELL OUT BETWEEN THE PINS. MASHED AND CUT TWO MIDDLE FINGERS ON LEFT HAND. EMPLOYEE WAS REPAIRING MINER - CAUGHT FINGERS BETWEEN ROCK AND MINER.
EMPLOYEE WAS CLEARING MATERIAL OFF OF A CONVEYOR FRAME WITH A SHOVEL. IT WAS CAUGHT BETWEEN THE BELT AND AN IDLER ROLLER WHICH CAUSED THE WORKER'S LEFT HAND TO HIT THE CONVEYOR FRAME.
INJURED WAS INVOLVED IN A BI-ANNUAL MSHA INSPECTION IN THE BASEMENT OF THE PLANT PUMPHOUSE WHEN HE SLIPPED ON MUD TWISTING HIS KNEE. AS HE ATTEMPTED TO CATCH HIMSELF, HE ALSO STRAINED HIS SHOU LDER. DID NOT BECOME REPORTABLE UNTIL HE STARTEDMISSING WORK DUE TO SURGERY ON 12/21/01. HAS RETURNED TO WORK, BUT IS SCHEDULED FOR KNEE SURGERYON 1/18/02.
EMPLOYEE WAS TRYING TO REMOVE A PIN FROM THE PNEUMATIC HAMMER ATTACHMENT ON A TRACK HOE. THE EMPLOYEE STRUCK THE PIN WITH A PIN HE WAS HOLDING. THE FORCE CAUSED A STRAIN ON HIS SHOULDER RESULT ING IN A PARTIAL TEAR OF THE SUBSCAPULARIS TENDON. EMPLOYEE DID NOT START MISSING TIME UNTIL 08/01/01.
DURING BURNER INSPECTION-LADDER RUNG BROKE, CAUSING EE TO FALL APPROX. 5 FEET.
INJURED STRAINED HIS RIGHT LOWER CALF MUSCLE WHEN HIS FOOT SLIPPED ON A SMALL ROCK (5/8"") WHILE WALKING UP AN INCLINED WALKWAY NEAR #4 SCREEN.
EMPLOYEE WAS REMOVING A PIECE OF SLATE FROM THE CONVEYOR. THE CONVEYOR WAS DE-ENERGIZED, BUT WHEN EMPLOYEE GRABBED THE SLATE, SOME COAL ROLLED DOWN THE CONVEYOR, STIKING HIS HAND.
EE WAS CHECKING ROCK BIN AND HIT HIS HEAD ON OVERHEAD PIPE.
EMPLOYEE STATED THAT HE WAS DUMPING HIS LOAD AT THE DUMP AREA WHEN THE BED OF TRUCK STARTED DOWN IT JARRED HIM IN THE SEAT CAUSING PAIN IN HIS GROIN AREA.
INJURED STATED THAT HE WAS WALKING, A SMALL PIECE OF ROCK FELL FROM THE ROOF STRIKING THE SIDE OF HIS RIGHT FOOT. CONTINUED WORKING, DID NOT TELL FOREMAN. CALLED MINE ON 5-31 AT 10:00 AM AND SAID HE HAD BEEN TO THE DOCTOR AND WAS GOING TO BE OFF FOR A FEW DAYS.
6 NORTH BELT, INJURED WAS HELPING ADVANCE THE BELT, A BELT CRADLE FELL FROM POSITION STRIKING HIM ON THE LEFT INDEX AND MIDDLE FINGERS, PINCHING THEM BETWEEN THE CRADLE AND BED RAIL.
WHILE DRIVING A RAMCAR, EMPLOYEE HIT A HOLE IN THE ROADWAY AND JAMMED HIS NECK.
WHILE TRAMING SCOOP TO CHARGER RAN OVER A ROCK CAUSING OPERATOR TO JAM HEAD ON ROOF.
THREE EMPLOYEES WERE INSTALLING A 25 HP ELECTRIC MOTOR ON THE PREP PLANT. THE MOTOR WAS BEING POSITIONED SO THAT IT COULD BE BOLTED DOWN. THIS EMPLOYEE HAD HIS RIGHT ARM BETWEEN THE MOTOR AND THE SHAFT. THE MOTOR STRUCK HIS RIGHT ARM BELOW THE ELBOW. CONTUSION, BRUISING AND SOME SWELLING RESULTED.
LOADER RAN INTO THE FRONT OF WATERTRUCK CRUSHING IT IN. WATERTRUCK DRIVER WAS ABLE TO GET HIMSELF OUT OF WATERTRUCK. EE WAS TAKEN TO EMERGENCY CENTER AND CHECKED BY DR, DR RELEASED EE TO RETUR N TO WORK AS HE FOUND NO INJURIES. NO X RAYS OR PRESCRIPTIONS WERE ISSUED. 3 WEEK 6-18-01 AFTER THE INCIDENT THE EE STARTED EXPERIENCING PAIN AND RETURNED TO THE DR FOR A FOLLOW UP. DR ORDERE
WHILE CLEANING OUT THE SCREENS, HE WAS ON HIS STOMACH AND CRAWLING, WHEN HIS BACK HIT A RUBBER ROLL. HE CONTINUED TO WORK AND HIS BACK PAIN GOT WO5RSE. HE WENT TO THE DOCTOR THE NEXT MORNING.
MASHED AND CUT LEFT HAND MIDDLE FINGER.
UP FRONT CHECKING OIL, SLIPPED OFF BUMPER & LANDED ON LEFT SHOULDER, STRAINED LIGAMENTS & BAD BRUISE.
WHILE LOADING A SCOOP BUCKET WITH ROOF SUPPORT MATERIAL. A ROOF CAP 36"" ROUND AND WEIGHING APPROX 15 LB FELL FROM HIS WAIST AND STRUCK HIS OUTSIDE LEFT FOOT RESULTING IN BREAKING HIS 5TH METAT ARSAL OF THE LEFT FOOT.
INJURED ROOF DRILL OPERATOR WAS ASSISTING THE OFFSIDE DRILLER IN STRAIGHTENING A BEND ROOF BOLT WHEN THE INJURED'S LEFT WRIST WAS STRUCK BY THE ROOF BOLT SPINNING.
EE WAS DRILLING ON THE BENCH FOR SOME UNKNOWN REASON THE DRILL RAISED UP ABOUT 10' AND FELL ONTO ITS RIGHT SIDE. EE JUMPED FROM THE DRILL, HAD THE WIND KNOCKED OUT OF HIM AND BRUISES.
EE WAS OPERATING A ROOF BOLTING MACHINE, HE WAS STARTING THE DRILL STEEL INTO THE MINE ROOF TO DRILL A HOLE FOR ROOF BOLT INSTALLATION. AS HE STARTED DRILLING THE HOLE THE IMMEDIATE ROOF BROKE FROM AROUND THE DRILL STEEL AND ONE PIECE OF ROCK FELL STRIKING HIM ON THE L. FOOT CAUSINGA BRUISE AND HEMATOMA TO THE TOP OF HIS L. FOOT.
EE WAS LEANING ON BOOM, UNKNOWN TO OPERATOR. OPERATOR TURNED MINER WHICH CAUSED BOOM TO SWING & PINCH EE BETWEEN BOOM AND COAL RIB.
WHILE UNLOADING ROLLERS FROM THE BACK OF A TRUCK, A ROLLER WAS REMOVED, WHICH CAUSED ANOTHER ROLLER TO MOVE ACROSS THE EE'S FINGERS, CAUSING SWELLING.
WORKING ON BELT SECTION FEEDER AND TWISTED HIS BACK.
WAS WASHING ON JIG FLOOR, SLIPPED AND TWISTED RIGHT KNEE. STARTED MISSING WORK ON 8-10-01.
THE EE WAS MOVING BUGGY ANCHOR WHEN HE LACERATED RIGHT HAND, REQUIRING 16 SUTURES ON A PIECE OF WIRE THAT WAS TIED TO A ROOF BOLT PLATE.
EE WAS REMOVING PIN FROM DUMP TRUCK TAILGATE TO UNLOAD SAND AND WHEN HE PULLED PIN OUT THE TAILGATE PULLED LOOSE AND MASHED HIS FINGER(INDEX) ON RIGHT HAND BETWEEN LATCH AND TAILGATE.
PINCHED FINGER WHILE CHANGING LEFT FRONT LIFTINGCYLINDER ON MINER.
EMPLOYEE WAS REINSTALLING ENGINE AND HAD A PAIN IN LOW BACK, IT STARTED AS DULL AND PROGRESSIVELY GOT WORSE. CAN NOT DETERMINE EXACTLY WHAT CAUSED THIS PAIN.
EE STATED THAT WHILE HELPING UNLOAD A DEWATERING PUMP, THE PUMP ROLLED AGAINST HIS RIGHT KNEE AND ANKLE.\
EE DID NOT KNOW WHAT HE DID, OR HOW HE INJURED HIS KNEE.
HE WAS HELPING DRILL HOLES IN THE BOTTOM TO ANCHOR TAIL PIECE. HE WENT TO MOVE DRILL AND HIT LEVER THAT RUNS DRILL. IT CAUGHT HIS HAND AND STRUCK THE NAIL THAT HOLDE BARREL ON DRILL THRU HIS R ING FINGER, CATCHING HIS RING AND LEDGING IN HIS FINGER.
EE WAS REMOVING A HOIST MOTOR COUPLING GUARD TO GREASE THE COUPLING. HE HAD TO TUG ON THE GUARD TO GET IT LOOSE. HE THEN STEPPED BACK AND TURNEDTO HIS LEFT TO SET IT DOWN. WHEN HE WENT TO STRA IGHTEN BACK UP HE FELT LOWER BACK LOCK UP & HIS LEFT LEG GO NUMB.
EE STATED HE WAS THROWING THE SWITCH ON THE 6 LEFT SPUR TRACK WHEN HE FELT A PAIN IN HIS LOWER BACK.

LOOSE FITTING GLOVES MAY HAVE BEEN A FACTOR.

AFTER FILLING WATER TRUCK, EE WENT TO GET INTO THE TRUCK AND SLIPPED IN THE MUD. HE WENT TO DOCTOR ON 6/4/01. DOCTOR SAID HE WAS FINE. ON 6/21 AND 8/8 HE WENT BACK TO THE DOCTOR AND WAS DIAGNO SED AS HAVING A HERNIA. THE COMPANY WAS NOTIFIED ON 12/4/ BY THE DOCTORS OFFICE THAT THE EVENTS HAD OCCURRED. AS OF 12/10 EMPLOYEE HAS NOT NOTIFIED EMPLOYER.
EMPLOYEE WAS CHECKING THE ANTI-FREEZE FLUID LEVEL ON A BULL DOZER. HE OPENED THE CAP TO RELIEVE PRESSURE. THE RADIATOR THEN SPURTED OUT HOT LIQUID AND SCALDED HIS HAND. HE WAS WEARING GLOVES.
WAS REPAIRING A TAIL SHAFT OM A FEEDER. HE WAS IN A KNEELING POSITION INSIDE FEEDER USING A RATCHET TO TIGHTEN BOLT. AS HE REACHED HE FELT PAIN IN HIS ABDOMEN. HE WAS DIAGNOSED WITH AN UMBILIC AL HERNIA AND BEGAN MISSING WORK ON 5-29, WHEN HE HAD SURGERY TO REPAIR HERNIA.
EE WAS ASSISTING A COWORKER TO REMOVE A RUB RAIL FROM A CONTINUOUS MINER. WHEN HE SWUNG THE RAILHE TRIPPED AND FELL STRADDLING THE RAIL. HE STATES HE LANDED ON HIS TESTICLES. HE WORKED UNTIL 6-5-01 ATWHICH TIME HE WAS INSTRUCTED TO REMAIN OFF WORK UNTIL FURTHER NOTICE.
EMPLOYEE AND FELLOW WORKER WERE REMOVING RADIATOR FROM LOADER WHEN THE RADIATOR DROPPED SLIGHTLY, MASHING THE TIP OF EMPLOYEE'S FINGER BETWEEN THE RADIATOR MOUNT AND LOADER FRAME.
EE WAS RUNNING SHUTTLE CAR. HE WAS BACKING DOWN AN INCLINE AND HIT A HOLE, JAMMING HIS HEAD UP AGAINST THE CANOPY OF THE SHUTTLE CAR.

THERE WERE NO LOST DAYS, HOWEVER THE INJURY IS NOW CAUSING SEVERE PAIN AND THE DR. IS RECOMMENDING SURGERY TO RECONSTRUCT THE LIGAMENTS
OPERATING 777 ROCK TRUCK & STARTED HAVING CHEST PAINS.
EE WAS SHOVELING GOB FROM TAILPIECE WHEN HE ALLEGEDLY HURT HIS BACK.

WHILE DRILLING A HOLE FOR A ROOF BOLT, A BOLT HOLDING THE OPERATOR'S CANOPY BROKE ALLOWING THE CANOPY TO DROP, STRIKING THE EE ON THE RIGHT SHOULDER. EE FIRST SOUGHT MEDICAL ATTENTION ON 7-18- 01 FIRST DAY OFF WORK WAS 723-01.

EE WAS PERFORMING CLEANING DUTIES WITH ANIRON BAR ON THE SECOND FLOOR AT THE CRUSHING PLANT. ACCIDENTALLY HE HIT A HANDRAIL WITH HIS LEFT LITTLE FINGER CAUSING HIM AN OPEN WOUND.

EE WAS ATTEMPTING TO INSTALL AN ELEVATOR BELT WHILE WORKING FROM A PLATFORM. THE HANDRAILS HAD BEEN REMOVED IN ORDER TO INSTALL THE ELEVATOR BELT. THE PLATFORM WAS APPROX 3 FT ABOVE GROUND. TH E EE'S RIGHT KNEE SUDDENLY GAVE OUT & HE FELL STRIKING RT KNEE ON CONCRETE PAD.
SUBJECT WALKING BACKWARD, INSTRUCTING DRIVER OF DRILL. STEPPED INTO A HOLE & INJURED RIGHT KNEE.
HE WAS HELPING MAKE A BELT SPLICE & GOT HIS LEFTTHUMB CAUGHT IN THE COME-A-LONG RATCHET BEING USED TO PULL THE TWO ENDS OF THE BELT TOGETHER.
REPETIVE MOTION INJURY. HE HAD BEEN DOING A LOT OF SHOVELING AND HAS NOTICED INCREASING PAIN. PAIN LESSENED AND FINALLYIN OCTOBER, 2001 HE HAD SURGERY.
EE WAS CLIMBING OVER STEEL BEAMS TO REMOVE PLUG ON DRUM WHEN HE SLIPPED ON WET MAGNATITE AND TWISTED HIS LEFT KNEE. EE WORKED FULL DUTY UNTIL 11-21-01. FIRST DAY OFF WORK WAS 11-23-01 DUE TO S URGERY.
WORKING ON CRUSHER UNIT, FELL BACKWARDS LANDING ON WRIST. BROKE WRIST.
EE HIT HIS KNEE ON THE RIPPER OF D-21 DOZER. HE CONTINUED TO WORK W/O COMPLICATION. ON 5-21 HE REPORTED THE INCIDENT TO HIS FOREMAN. ON 5-22 THE EE WENT TO HOSPITAL FOR XRAYS. DR. SAID XRAYS G OOD, NEED TO STAY OFF KNEE AND ALLOW THE SWELLING TO GO DOWN.
NECK WENT OUT OF PLACE WHILE SHOVELING.
EMPLOYEE WAS ON THE LEFT SIDE OF THE CONTINUOUS MINER WHEN A PIECE OF LOOSE RIB COAL FELL CAUSING HIS LEFT LEG TO BE CAUGHT BETWEEN THE LOOSE COAL AND THE CONTINUOUS MINER MACHINE.

WAS ON SMALL BANK WELDING ON JAW CRUSHER, BRACING SELF WITH LEFT ARM, WELDING FOR A LONG PERIOD OF TIME AND GOT BURNED FROM THE LIGHT OFF THE WELDER.
EE WAS GRINDING IN A REAL TIGHT SPOT AND THE GRINDER (9"" HAND HELD GRINDER) KICKED BACK AND HIT MY LEFT KNEE. SOME SWELLING AND HURTS WHEN HE BENDS THE KNEE BUT IF HE STANDS UP STRAIGHT DOES N OT HURT.
EE WAS UNLOADING SUPPLY'S FROM A FLAT CAR WHEN A ROCK 3 FT X 4 FT BY 10 IN. FELL FROM BETWEEN THE BOLTS STRIKING HIM ON THE FOOT CAUSING THE INJRY.
EMPLOYEE SLIPPED AND FELL IN A PUDDLE OF OIL IN THE FUEL BAY.
CREW WAS TAKING DOWN K DOORS. INJURED HAD HIS HAND ON TOP OF DOOR. WHEN DOOR POPPED UP HIS RING FINGER WAS CAUGHT BETWEEN DOOR AND ROOF CAUSING A SMASHED LEFT RING FINGER REQUIRING STITCHES.
INJURED WAS CHIPPING NORDBAK ON TRUNNION PANEL AND FELT SOMETHING FOREIGN INHIS LEFT EYE. (EE MISSED THE 23, 24TH OF MAY).
WHILE WORKING ON A WELDER, HE FELT HE REINJURED HIS BACK AFTER A FEW DAYS IT DIDN'T GET ANY BETTER SO HE SAW HIS DOC WHO SENT HIM TO A SPECIALIST. WENT TO DOC 5-24-01. OFF WORK UNTIL NEXT APPO INTMENT 6-4-01.
EE WAS EXITING OPERATOR CAB OF BOBCAT, STEPPED FROM BUCKET TO THE GROUND AND ROLLED HIS ANKLE. HE CONTINUED WORKING UNTIL 11/1/01. HE WENT TO DOCTOR BECAUSE OF CONTINUING PAIN AND TORN TENDON WAS DIAGNOSED. SURGERY PERFORMED 11/17/01. FIRSTDAY OF LOST TIME 11/19/01.
EE WAS CLIMBING UP STEPS TO 980C LOADER. HE WAS CARRYING GREASE GUN AND 3 TUBES OF GREASE. ON 2ND STEP UP HE LOST HIS BALANCE AND FELL BACK LANDING ON HIS DERRIERE. THE NEXT DAY HE HAD OCCATIO NAL BACK PAIN AND THE 2ND DAY (SAT) HE WAS UNABLE TO WALK COMFORTABLY.
EMPLOYEE WAS SHOVELING PONTOONS, THROWING THE LOAD OVER HIS LEFT SHOULDER WHEN HE FELT A POP IN HIS SHOULDER.
EMPLOYEE WAS AT TOP OF STAIRS IN CORE BUILDING. HE TURNED TO SEE WHAT WAS GOING ON DOWN THE MIDDLE LANDING OF THE STAIRWAY. AS HE TURNED HE TWISTED HIS ANKLE AND HEARD A LOUD SNAP.
WHILE STANDNG ON A 8'LADDER INSTALLING CURVED SHEET METAL AND PANEL. THE PANEL WAS BEING LOWEREDINTO PLACE, LINE UP BARS WERE INSTALLED AND PANEL COULD NOT BE HANDLED BY HIS END, IT SLIPPED CO MING DOWN AND STRUCK HIM ON RIGHT ARM.

EE SUSTAINED FRACTURES AND LACERATIONS TO RIGHT HAND.
THE INJURED EE WAS PERFORMING MANUAL MATERIAL HANDLING, UTILIZING A SHOVEL AND WHEELBARROW. WHILE FILLING THE WHEELBARROW, HE HIT HIS HAND ON THE WALL CAUSING THE WHEELBARROW TO TILT. IN TRYIN G TO PREVENT THE WHEELBARROW FROM TIPPING OVER, HE PULLED A GROIN MUSCLE RESULTING IN AN INGUINAL HERNIA ON THE RIGHT SIDE.
OPERATING ROOF BOLTER AND LET THE DRILL POT DOWN ON HIS KNEE.
WAS SHOVELING AT PLANT & FELT A PULLING IN STOMACH.
EE WAS TAKING DOWN A BACKUP CURTAIN TO MOVE UP, SAW THE SHUTTLE CAR COMING AND THOUGHT THE DRIVER SEEN HIM. CAR STRUCK EE ON LEFT LEG.
EMPLOYEE FAILED TO USE PROPER FALL PROTECTION. WHILE HELPING REPLACE A BURNT OUT ELECTRIC MOTOR, HE FELL STRIKING THE BED OF MACK DUMP TRUCK, INFLICTING MULTIPLE FACIAL LACERATIONS THAT REQUIRED STITCHES.
EMPLOYEE WAS OBSERVING OPERATION OF EUCLID END-DUMP FROM PASSENGER SEAT. THE END-DUMP OPERATOR BACKED INTO RETAINING WALL OF LOAD OUT BIN CAUSING EMPLOYEE TO STRAIN HIS NECK.
EE WAS PULLING ON CUTTING MACHINE CABLE, HE PULLED A MUSCLE IN THE UPPER RT. ARM.
SHOVELING WET MATERIAL ONTO STOPPED MINE BELT CONVEYOR. PULLED MUSCLE ON RIGHT SIDE RIB AREA.
THE INJURED WAS REPLACING THE BOLTER AND STANDING ON THE CABLE WHEN THE REEL STARTED TAKING UP THE CABLE, IT CAUGHT HIS LEG AND CAUSED HIM TO FALL. THE INJURY WAS DIAGNOSED AS A STRAIN.
THE INJURED EMPLOYEE WAS IN THE PROCESS OF PERFORMING MAINTENANCE ON THE #1 BELT LINE WHEN THE INJURY OCCURRED. HIS RIGHT HAND WAS CAUGHT BETWEEN TWO PIECES OF METAL CAUSING THE INJURY.
EE WAS RUNNING THE LEFT SIDE OF DOUBLE HEAD BOLTER WHEN THE RIGHT SIDE OPERATOR WENT TO SWING HIS HEAD. THE STAB JACK WAS NOT ALL THE WAY UP. ITCAUGHT ON MINE FLOOR CAUSING THE BOLTER TO MOVE FORWARD. THE LEFT RIB MASHED THE LEFT SIDE OPERATOR'S LEG BETWEEN THE HEAD AND RIB.
EE WAS UNLOADING BELT VULCANIZING EQUIPMENT OFF A FLATBED TRUCK IN THE SURFACE SUPPLY YARD. WHENHE PICKED UP A SECTION OF THE EQUIPMENT HE FELT PAIN IN HIS GROIN AREA.
EE WAS USING A DRILL. THE DRILL BIT BECAME STUCK AND THE HANDLE OF THE DRILL TWISTED RESULTING IN EE'S HAND BEING FRACTURED.
EMPLOYEE WAS HELPING ON A ROOF BOLTER WHEN HE PASSED OUT AND FELL CAUSING HIM TO INJURE HIS RIGHT HAND. HIS HAND CONTINUED TO HURT AND ON 5-18-01 THE EMPLOYEE BEGAN TO MISS WORK.
EE WAS WALKING ALONG THE BELT AT THE 29W MN 1ST BELT TAILPIECE. A WATERLINE HANGER ABOVE HIM CAME LOOSE AND ALLOWED THE 4"" WATER LINE TO FALL. IT STRUCK THE EE ON THE TOP OF HIS LEFT SHOULDER. THERE WAS NO DAMAGE OR IMPAIRMENT TO MINING EQUIPMENT OR THE MINING OPERATION. DID NOT INVOLVE COMPLIANCE, JOB SKILLS, PROFICIENCY, TRAINING, ATTITUDE, PROTECTIVE ITEMS OR PROTECTIVE DEVICES.
EMPLOYEE WAS WALKING ALONG THE TRACK. HIS FOOT SLIPPED AND GOT CAUGHT BETWEEN THE RAIL AND A TIE. THE EMPLOYEE HIT HIS HEAD ON THE ROOF, FELL, AND TWISTED HIS ANKLE.
INJURED WAS TAKING DISCHARGE LINE APART WHEN PIPE SPRUNG BACK STRIKING INJURED ON LEFT SHOULDER CAUSING A BRUISE.
2ND LEFT BELT DRIVE INSTALLATION, POWER CABLE FOR BELT DRIVE MOTOR FELL OUT OF LANGERS AND STRUCK EMPLOYEE ON BODY, LEFT WRIST, AND LEFT SIDE OF HEAD. EMPLOYEE WAS BENT OVER UNDERNEATH THE CABLE HELPING TO WORK ON BELT GUARD.
EE WAS BENDING A 6' BOLT WHEN HE EXPERIENCED PAIN IN HIS RIGHT WRIST. HE FINISHED HIS SHIFT, WORKED NEXT DAY UNTIL 9:30AM AND WENT TO DOCTOR. HECALLED IN 5/18. HE WOULD NOT BE IN TO WORK.
EE AND OTHER WERE WORKING ON A MACHINE AND HE GOT HIS LEFT PINCKY FINGER CAUGHT HE CUT THE TIP OFF.
EMPLOYEE WAS IN THE PROCESS OF CHANGING A 12"" IN THE PREP PLANT. WHILE POSITIONING THE PIPE THE EYE BOLT USED TO HOLD THE PIPE CAME LOOSE ALLOWING THE PIPE TO MOVE STRIKING THE EMPLOYEE'S RIGH T LEG, JUST ABOVE THE KNEE.
AFTER COMPLETEING HIS SHIFT EE WAS WALKING IN THE PARKING LOT WHEN HIS ANKLE GAVE WAY FOR NO APPARENT REASON.
EMPLOYEE WAS WALKING TOWARD OUTSIDE OF MINE. HE SLID IN A SLICK PLACE AND FELL FORWARD. HE BROKE A FRONT TOOTH AND DAMAGED OTHER TEETH.
EMPLOYEE RECEIVED LUMBAR SUPPORT WHEN FOOT SLIPPED OFF THE LADDER WHILE DISMOUNTING THE HAUL UNIT.
EE STATES THAT HE WAS REPAIRING A SHAKER AT THE QUARRY. AS HE STEPPED DOWN FROM THE MACHINE, HE TWISTED HIS RIGHT KNEE.

EE WAS UNLOADING AMMONIUM NITRATE INTO BIN WHEN LIGHTING STRUCK THE GROUND(500' EST) FROM BIN. EMPLOYEE REACHED TO TURN PRESSURE GAUGE ON TRUCK AND RECEIVED ELECTRICAL SHOCK FROM HOLDING PRESS URE GAUGE.
WHILE EMPLOYEE WAS PINNING THE FACE, A ROCK ROLLED OFF THE FACE AREA PINNING HIS LOWER RIGHT LEGTO THE FOOT JACK. HE ALSO HIT HIS MOUTH (TOOTH) ON THE PIN.

WHILE CRAWLING IN LOW COAL EMPLOYEE HAD WRIST BENT BACK, CAUSING SPRAIN WITH LIGAMENT AND TENDONDAMAGE. ORIGINALLY HE DID NOT MISS ANY WORK. AFTER FURTHER EXAMINATION OF WRIST, HE WILL BE OFF FOR AN UNDETERMINED AMOUNT OF TIME.
GREASING TAIL ROLLER, WHEN THE GUARD OF THE TAILROLLER FELL STRIKING HIS SHOULDER
EE WAS LOWERING DRILL STEEL INTO HOLE. IT STARTED TO GO FAST SO HE GRABBED IT. A SUDDEN JERK PULLED HIS RIGHT SHOULDER OUT OF PLACE CAUSING PAIN. EE WAS UNABLE TO WORK ONE DAY BECAUSE OF THIS.
EE WAS OPERATING MACHINE TOO CLOSE TO EMBANKMENT. THE EE WAS NOT WATCHING EMBANKMENT WH IT COLLAPSED ONTO MACHINE. EE WAS PINNED TEMPORARILY BY HIS FEET. EE THEN DISEMBANKED MACHINE AND WAS TA KEN TO HOSPITAL WHERE HEWAS TREATED FOR BRUISE AND RELEASED.
EMPLOYEE HAD FINISHED SERVICING ON END LOADER. AS HE WAS WALKING HE TWISTED HIS KNEE. HE CONTINUED TO WORK UNTIL JUNE 14, 2001 WHEN HE BEGAN LOSING TIME.

HE DEVELOPED PAIN IN RIGHT ARM WHILE LIFTING CEMENT BLOCKS TO BUILD A BRATTICE WALL.
THE INJURED EE WAS PULLING ON A WATER LINE. AS THE EE PULLED ON WATER LINE, HE PULLED A MUSCLE IN LOWER BACK. THE FIRST DAY THE EE MISSED WORK WAS 5-24-01.
EE WAS REMOVING ROCK FROM 1415 CM MINER WHEN A PIECE OF ROCK SLIPPED CATCHING HIS RIGHT THUMB AND INDEX FINGER BETWEEN THE MINER AND ROCK.
2 TRACK RAILS FELL OFF RAIL CAR AND LANDED ON EMPLOYEE'S LEFT SHIN AND FOOT.

EMPLOYEE WRENCHED HIS KNEE WHEN HE SLIPPED, CLIMBING ONTO HIS SERVICE TRUCK.

WHILE CHECKING PACKING ON CYCLONE PUMP EE AS ACCESSING THE PUMP & SLIPPED IN MUD THAT WAS DEPOSITED FROM LEAKING & OVERFLOWING COBBER TAILS LAUNDERS OVERHEAD. SURGERY WAS PERFORMED N 4/3/02
EE SLIPPED ON DRILL PLANK WHEN WALKING AND TWISTED R KNEE.
FEEDER FLIGHT CAME OUT OF THE FEEDER AND HIT EE ACROSS LEFT SHOULDER AND CHEST.

THERE WAS NO SPECIFIC EVENT REPORTED THAT CAUSED THE INJURY. EE'S REGULAR JOB DUTY IS AS END DUMP DRIVER, BUT HE WAS TEMPORARY PLACED AS A FUELER TO HELP OUT THE MAINTENANCE DEPT. THE PERSISTE NT MOUNTING & DISMOUNTING EQUIPMENT IRRITATED HIS LEFT KNEE WHICH CAUSED PAIN AND SWELLING. HE WAS DIAGNOSED TO HAVE A SPRAINED KNEE.
USING WOMA GUN, EMPLOYEE PLACED HIMSELF IN AWKWARD POSITION, MAKING HIM UNABLE TO CONTROL WOMA GUN.(PREVIOUS INJURY 2/12/00-USING WOMA GUN).ON 4/09/01 EMPLOYEE AGGRAVATED OLD INJURY, RESULTING IN LOST TIME 5/25/01. THIS REPORT STEMS FROM THE INCIDENT. EMPLOYEE NOW FILING ANOTHER REPORT BECAUSE OF PAIN IN NECK AS WELL AS IN SHOULDER. LOST TIME ATTRIBUTED TO 5/14/01, NOT PRIOR INJURY

EE WAS OPERATING THE LEFT SIDE OF THE ROOF BOLT MACHINE & INSTALLING THE OUTSIDE BOLT STANDING NEAR THE RIB LINE WHEN A PIECE OF ROCK MEASURING 4' X 8"" X 8"" FELL OUT FROM THE COAL RIB STRIKING HIM ON THE RIGHT LOWER LEG. THE ROCK CAME FROM APARTING LOCATED IN THE COAL SEAM ~3' FROM THE MINE FLOOR. THE COAL HEIGHT IN THE AREA WAS 7' TO 8'. EE STATED HE DID NOT DETECT THE LOOSE ROCK.

AS THE OPERATOR STARTED THE ROOF BOLTING MACHINE THE TRAIL ING CABLE JERKED JAMMING THE EMPLOYEE'S RIGHT THUMB. THIS INCIDENT IS PENDING INVESTIGATION DUE TO MEDICAL RECORDS.
EE WAS WELDING W/ARCH WELDER HE STOPPED WELDING, RAISED HOOD AND BEGAN PRYING ON PLAT S/PRY BAR. THE PRY BAR SLIPPED, STRIKING EE IN EYE. SAFETY GLASSES WERE PROVIDED TO EE PRIOR TO ACCIDENT.
EE WAS ON THE LONGWALL FACE USING A COME-A-LONG TO PULL THE FACE CHAIN BACK TOGETHER. AS HE PULLED ON THE COME-A-LONG HANDLE HE FELT A BURNING SENSATION IN HIS ABDOMEN. THERE WAS NO DAMAGE OR IMPAIRMENT TO THE MINING OPERATION OR EQUIPMENT.PROPER USE OF THE TOOL WAS A FACTOR. THE INJURY DID NOT INVOLVE JOB SKILLS TRAINING OR ATTITUDE.PROTECTIVE ITEMS OR DEVICES NOT A FACTOR.
EE WAS PULLING ON A ROLLER TO GET IT OUT FROM BETWEEN TOP AND BOTTOM BELTS TO HANG IT ON THE TUBES OF FRAMING. HE TURNED AND LIFTED AT THE SAME TIME WHEN HE EXPERIENCED LOWER BACK PAIN.
EE TIGHTENING BOLTS ON FORM DOOR WRENCH SLIPPED-HURTING SHOULDER.
EMPLOYEE DROPPED THE CONTINUOUS MINER RUB RAIL GUARD ON HIS RIGHT FOOT.
CLIMBING DOWN A LADDER, HE SLIPPED OFF RUNG CATCHING HIMSELF WITH LEFT ARM.
EE CLIMBED ON VERY SMALL STEP LADDER TO CHECK LEVEL OF HYDRAULIC OIL ON HAUL TRUCK. HE FELL FROM LADDER LANDING ON SHOULDER. HE DIDN'T THINK IT WAS A REPORTABLE ACCIDENT, BUT HIS SHOULDER CONT INUED TO HURT AND HE SOUGHT MEDICAL TREATMENT.
STRAIN/SPRAIN LEFT SHOULDER BACKFILLING SHOT HOLES.
INDIVIDUAL HAD COMPLETED MINING CROSSCUT AND HAD MADE AN INITIAL CLEAN UP PASS ON THE OPERATOR SIDE OF THE CROSSCUT. INDIVIDUAL PIVOTED MACHINE TO CLEAN OPPOSITE SIDE OF CROSSCUT AND THE RIB B SLOUGHED AND PINNED HIM AGAINST THE RIGHT REAR CORNER OF THE MACHINE.
EE WAS HELPING ALIGN BELT STRUCTURE AND HURT HIS BACK AS HE WAS PULLING ON STRUCTURE. EE STARTED MISSING WORK ON 5-14-2001.
