While just starting to mine in the F entry of 7 South 003-0, the miner cut into an uncharted oil or gas well. This well was uncased. There were no injuries with this accident.

Sparta Elevator was down at approximately 8:48 AM to 9:17 AM due to the PLC needing reset on the elevator.
A Non-Injury Roof Fall occurred on 2/27/17. The roof fall is located in the No. 3 entry at x-cut No. 15 on the 2-Left Panel off the 4th North Mains. The approx size of the roof fall is 40' in length, 18'-19' wide x 6'-8' thick. The MSHA Hot line was notified. Call conf. No. 3100034. The fall has been dangered off/wooded cribs installed on all sides of roof fall.
A roof fall was discovered at Break 14, 1-North, inby spad 205. The fall is approximately 20' x 20' 8' long. All approaches to fall is breakered off.
2/26 a ground fall occurred at 23E 300 A3E. 5' bolts of ground support system came out in the fall, 8' bolts of support system were still intact. A rehab plan was developed of 12' bolts & shotcrete. Rehab was started, ground started unraveling, releasing the 8' bolts, 12' bolts held. 2/27 management inspected area & called to hotline even though conditions didn't meet criteria.
Non emergency evacuation of the mine via the production hoist to examine the Koepe building. The mine will be idle until the evaluation and repairs are complete.
An electrical storm passed through the area causing the plant to drop power, it was found later that this event created potential to cause a coal dust ignition in the plant's coal mill. Damages were contained internally to the coal mill system. It was found that the damper plate in the outlet piping was damaged.
A coal smoker was discovered in an inactive pit that would not be extinguished within 30 minutes
A roof fall occurred above the anchorage point on the 11W set-up face. The passageway has been redirected through the adjacent entry and ventilation was not effected. The dimensions of the fall are approximately 23' wide X 23' high X 600' long. This was a combination of 3 falls that were continuous over approximately 3 days. No other 50.20 criteria applies.
Operator had just dumped the load and heard a loud thud and then fire came up around the cab, operator put machine in park and applied the parking brake and exited the machine. Fire was too big to fight so EE got away from it.
Passenger vehicle was parked on east end of crushing bench in an area away from normal equipment operations. During work on the crushing bench the WA-470 loader required fuel. Operator drove loader to fuel tank situated east of the passenger vehicle and fueled. After fueling the operator backed up striking the hood and grill of the vehicle.
IGNITION - Occurred on No. 7 section in the No. 4 entry. The Continuous Miner was cutting when the bottom got hard causing a frictional ignition lasting 5 to 10 seconds and was extinguished with a fire extinguisher.
Preheater elevator stuck between the 1st and 2nd floors with contractor inside for approx. 30 mins when MSHA was contacted. Elevator emergency brake faulted when someone attempted to open the door while elevator was in operation. Elevator brake was reset and car was brought to ground. No injury resulted.
The hot material started a fire on apron belt conveyor. The fire started around 23:00 conveyor stopped shortly after the fire started. Fire squads arrived at 23:20 and put out the fire within 90 minutes. At 00:30, the fire was out and the fire fighters remained on site to secure the area until 02:47.
No one currently works at this mine. A worker from another mine saw the smoke as EE was driving by. The 2 X 0 coal pile had a hot spot. There was no damage to any equipment and no one was injured.
Found an unintentional roof fall in the return adjacent to No. 3D Belt Drive in the 2nd Left Panel off Slone Mains.
Mucker operator heard pop, looked behind and saw a mist that erupted in flame, operator activated fire suppression system and shut master switch off. Traveled past mucker out to station and to surface. High pressure hydraulic line blew out on exhaust.
Three employees were trapped on the Blake's Ridge elevator at 11:25 am. The elevator was fixed and they returned to the surface safely.
A roof fall occurred on this date. The fall was located on 1 Right Panel off 3-East Sub Mains at Crosscut 27 in the #3 Entry. The fall measured 30' by 30' and was 10-12' in thickness. A mobile bridge unit was involved but there were no injuries. The area was previously supported with 8 and 12 foot cable bolts.
On February 18th at 18:08 the kiln coal mill was started with material being fed to the mill, the coal mill shutdown at 18:56 due to high vibration. The CO in the dust collector continued to climb until the time of the explosion.
A grass fire was started by the hot exhaust on a contractors pick-up. MSHA, fire department and shift supervisor were notified. A fire lane was plowed and the fire was extinguished.
AN UNPLANNED ROOF FALL OCCURRED AT XC128, ENTRY 5, ON THE 1ST NORTH SUBMAIN. THE FALL MEASURED 30' LONG X 19' WIDE X 7.5' THICK. THE AREA WAS BOLTED WITH 6' DOUBLE LOCK BOLTS. NO INJURIES WERE SUSTAINED, APPROX. 40' OF BELT STRUCTURE WAS DAMAGED.
An unintentional roof fall occurred in the 5L section in the return entry at approx. 94+92. The fall measured 25ft wide by 6ft high by 17ft wide. There was no injuries and no equipment damage as a result of this incident
EE was traveling the return and found a roof fall in the old south mains near spad number 7047. The fall was not in a regular travelway and no air restrictions were found. No accident or injuries were found. The fall was approx 80'x 19'x 4-8'thk.
On 2/15/2017 a roof fall was discovered at 3 west mains break 15 #1 entry where miners do not normally work or travel all approaches to this area have been dangered off.
PLC kicked power to the production hoist. The PLC contactors were checked, cleaned and then reset and tested. The repairs with testing was completed at 12:20am on 02/16/2017. During this time all employees were safely evacuated via the Keope Hoist.
2 unintentional roof falls were discovered during the weekly examiners route on 2/15/17 at approximately 13:50 pm. One fall in the #1 entry between xcuts 33 and 35 and the second one is in #2 entry between xcuts 36 and 37 in the 5th Left Panel off Dykes Branch Mains.
Employee fell approximately 5 feet when employee fell off a shovel bucket. Employee was not hurt
Intersection in #1 heading fell inby the tail piece over anchorage over the weekend while no one was underground.
Two contract mechanics were assigned to trim the teeth of the head sprocket to the ASF bucket elevator. The ASF system was offline at the time of incident. Sprocket bolt was cut with a torch, which fell down the ASF elevator shaft, igniting ASF material approximately half way down the shaft. The local fire department was called and fire was extinguished completely by approx. 13:00.
While mining a cut of coal in cross cut of room 26 to room 27 the rib blew out and there was an open hole approx. 2 1/2 feet in the rib line.
An unintentional roof fall occurred in the number one left return air course entry, out-by the MMU 003-0 / 004-0 working section, in-by survey station number 5788. The roof fall measured approximately 6'T X 35'L X 18'W.
Unplanned roof fall in the C-2 butt, room #2, 1 crosscut outby spad #608. At the time of the fall, the primary roof support was 4 ft. Fully grouted resin bolts.
An unplanned roof fall occurred on the #1 pillar section inby the loading point in the #4 entry 40' inby spad #12628. Measurements on the fall were 40' long x 30' wide x 4.5' to 8' high.
Ice built up on safety switch that slows down Miracle Run Elevator as it approaches the bottom allowing it to go into an over travel fault (arm of switch was unable to turn). One person was entrapped for approx. 5 minutes. Cause of incident was not determined until 2/10/2017.
The 3rd shuttle car out of the lift from the block was being loaded when the cars started loading. The miner back out of the place as the MRS were being moved. A slicken sided sandstone slip fell catching the MRS. The fall was approximately 60'L x 18'W x 8'T, wedge shaped
An unintentional roof fall has occurred on the P3-1 at the 16th XC in the #9 entry. The fall was approximately 20'X 20'X approximately 13'plus high
An employee damaged the service cage header while unloading materials. The employee tried to make the repairs but when unable, ee reported the incident to the Mill operator. The Mill operator relayed the message to the on shift maintenance supervisor. The supervisor traveled to the area, inspected the damage, and decided to down the hoist for repairs.
An unintentional roof fall occurred in the track entry, at #2 xc of the Cedar Grove coal seam.
On 2-8-17 a roof fall was discovered at 2 west mains break 45 #8 entry where miners do not normally work or travel all approaches to this fall have been dangered off.-
A unplanned fall of the roof occurred in the 5th South West Panel, crosscut 38 in the #00 entry, which is worked out area (old #4 unit). The fall measured approximately 15'-19' width, 7'-12' height, & 20'-30' in length.
The slope hoisting car that hauls men in and out of the mine went down. A bad battery was found to be the problem. The slope car was fixed at approximately 12:30am. Men were still able to get out of the mine and no portals were blocked.
#6 portal elevator went down at 10:00 am. The safety switch needed reset after a power outage. The elevator was running at 10:40 am.
The hoist at #1 slope went down at 9:00 am. The HMI screen was damaged due to a power outage. The HMI screen was replaced and the hoist was up and running at 10:25 am.
A roof fall has occurred inby the longwall face in the bleeder area.
An accidental/unplanned roof fall was discovered in an out/by area of the mine at # 114 A block F-Mains, in the track haulage entry. This roof fall area was subjected to constant temperature/ weather changes, and the immediate roof displayed water leakage in the fall area. There was no disruption to ventilation, no injuries, or equipment damage. Event was immediately reported.
There was a unplanned roof fall on 12-54C Belt, at spade 54+00.
Supervisors weekend pre-shift inspection, noticed a light plant on fire. 7 on mines on site Nearest dragline was.93 miles away and 8 miles away for help. Safety assisted over the phone. There was no other potential hazards due to location. Fire was put out with 150lbs and re-started then buried with dirt. Safety notified MSHA after knowing we had control of the fire.
An unintentional roof fall was found by weekly examiner in the Johnson Fork Mains at spad no. 7478 in the no. 7 entry beginning at x-cut 34 (inby approx 1/2 way of the block) and extending to the 36 x-cut, 20 feet wide and above the anchorage of the bolts.
The Hoist car was not operational for an extended period of time due to a broken Axle.
While in the process of installing chain hangers an employee drilled into a small pocket of methane. The employee stopped work and called the supervisor. All work was stopped in the unit, power was de-energized, ventilation was increased, and MSHA was notified.
Employee was last observed as employee began to ascend an extension ladder. There is no eye witness, it is apparent the employee fell approximately 20ft. onto a moving conveyor belt and was fatally injured.
A piece of draw slate approximately (3 foot by 2 foot by 3 inches thick) broke across the strap and fell striking ee. Injured miner was transported to a medical center. Due to medical complications from the injuries ee sustained ee died on 4/6/17
Truck bed turned over while dumping after load failed to clear from bed. This caused the truck to tip up on passenger side tires-truck did not turn over or sustain any damage. Material was wet and damp and temp. was cold. Driver chose not to use bed treatment station provided by company. Trailer appeared to be loaded toward the front. Driver chose to jump from truck.
Employee was operating a Getman tractor pushing a sled around the corner, employee grabbed the outside of the cab to pull self up in the cab seat. As the center section (pulling end) of the Getman contacted the cab (motor end), it smashed employee's hand between the two sections.
Replacing cylinder on loader. Strap on cylinder slipped mashing hand between cylinder and loader frame.
Miner climbed up stockpile to observe conditions of head roll on pea rock conveyor. Miner lost footing and right hand went through drive pulley pinch point, slicing three fingers through miners glove.
Employee was cutting metal with a grinder and disc shattered and penetrated right arm.
Injured was bolting roof on offside of machine, while ee was drilling a piece of rock 2 1/2"" thick x 30"" long x 20"" wide fell sriking ee on head and shoulder.
Employee was entering the garage, slipped, fell, scraped nose on Coumadin medication, and had to go to the ER to stop the bleeding.
Employee was taking an operator break when employee slipped on the third step of the power step on the haul truck, landing on employee's lower back and tailbone. Employee was taken to the ER, referred to employee's PCP, and taken off work until 3/5, missing employee's night shift of 2/28.
Employee was pushing bolt up into top when a knot popped up above groin area. Employee did not miss work until 03/03/2017.
Wet/muddy grinding wheel exploded resulting in loss of control of grinder
Employee was carrying a bottom belt roller when felt pain in lower back.
EE stepped off of 314 excavator and tripped and went backwards into metal door of grease truck striking self in testicles.
Employee was pulling/hanging miner cable in low coal conditions on-section. EE strained back, ee received medical attention at local ER. EE was released and told to follow up with primary care physician.
Hydraulic ram broke free from pressing pin out of bores on CAT 011R Dozer and struck employee on the head.
IE (Temp Employee) was changing the battery on the battery coal hauler. The plug wrench slipped and EE lost balance and fell onto outstretched left wrist. Fractured left wrist.
Injured was tramming track bolter from grassy towards page when employee struck something in the travel way at cross cut 43 on Grassy Creek Mains causing the bolter to suddenly stop causing the injured to hit employee's back resulting in a pain in employee's lower back.
Employee came in contact with a damaged area in the angle bolter cable and suffered an electrical shock which caused to fall to the ground catching self with right hand. Causing a fractured bone in right arm
Employee was working to align flange holes on an 18"" diameter pipe. Employee lost balance while coworker was driving a bull pin with a sledge hammer. Employee reached out to catch self and finger landed on the end of the bull pin right as the other employee was striking it. The weather was reported to be calm winds and approximately 45F.
In stowing the jib there is a safety pin that was not installed before removing the front pins at the boom.
Employee was struck in the arm with plow chain while assisting in putting the plow chain back together.
Associate was breaking in 50 lb bags at a rework station when the left hand cramped such that the associate was not able to open employee's hand.
The employee was installing a transmission guard on a piece of Caterpillar equipment and caught employee's right foot between the guard and frame. Employee broke bone in top of foot. Employee did not seek medical attention until 3-5-2017.
Employee went to step / dismount from Ford Dump Truck from drivers side while raining and slipped on the side of the step and twisted right ankle causing a sprain in that ankle. It was raining hard that day causing step area to be slick due to it being metal and wet.
Employee stated employee was riding in the back of a diesel mantrip when the mantrip hit an irregularity in the mine road causing employee to strain back. The result of the strain was lost work days.
Employee was pressure washing a 631-G Scraper, employee stepped off the fender and slipped on a spot of grease twisting employee's left ankle.
SLIPPED ON BLACK ICE - FRACTURED RIGHT FIBULA
Contractor was assisting mine operator in making modifications to the primary wash plant which involved removal of water pipe. Contractor installed rigging to hold pipe contractor was going to cut. As contractor completed cut, pipe shifted in the rigging allowing it to swing striking contractor on left side.
Backing over material/rocks repeatedly and jarring back.
Employee was using a hammer to break up rock in the grizzly bars and a rock came back and struck safety glasses, and injured eye.
Employee came down after a few mins not knowing a pry bar was sticking up from the ground. Employee leaped off the second step and fell back (due to ground level) into pry bar and cutting inner thigh. Slipped into rectum, aprox. 8 inches, tearing large intestine, and puncturing gall bladder.
Employee was walking to the long wall face when stepped on a rock and twisted right ankle. Did not start missing time from the injury until 04/10/2017.
Fresh water line blew apart and struck employee on shoulder. Continued to work unit 5/10/2017. Had surgery on 5/10/2017
Employee entered the 2nd floor Preheater MCC to work on the feed shelf analyzer. When employee went in, the room was hot because the air conditioner wasn't operating. Employee went to the bucket to put power back on the AC breaker and it exploded.
Employee was walking down the slope belt. Employee turned sideways and stepped over a hose and felt a pain in employee's left knee. Diagnosed with a torn meniscus on 3/7/17.
Employee was examining 3 belt around 48 wall when employee took a step and strained employee's left knee. The walkway in the area was level and had no obstructions. Employee was diagnosed with a torn ACL and began missing work on 5/5/17 to have employee's knee surgically repaired.
A fully trained miner was injured when EE tripped on a piece of rail causing EE to jam right shoulder and neck. The accident was not a result of any compliance issues nor were any protective guards involved.
Driver (contractor) was on top of load trying to level off tarp when employee lost employee's balance & fell.
Employee crashed vehicle on ice after shift on way home.
While roof bolting the two drill steels cross threaded and popped apart. One section of the drill steel hit employee's left hand.
EE was picking up a piece of metal and felt something pull in their lower back
Miner was pushing rock with a dozer when the blade hit a big rock causing the dozer to stop and tighten the seat belt straining employee's back.
Employee was getting off of a 657B Cat scraper when EE slipped and fell landing on the left knee.
Employee was cleaning snow out of A switch in the Solvay landing rail yard & stepped in between the bridle rod and felt knee hyperextend and immediately felt swelling. Employee informed supervisor of the incident at approx. 12pm the same day.
Employee was doing pre-shift inspection of PC 4000. Tripped in engine compartment and struck their ribs on firewall.
Miner was using sledge hammer on coal chute inside Coal Preparation Plant when the chute was missed with hammer head. Miners hand struck chute causing fracture to Left index finger and laceration to said finger.
Employees were loading mono-rail cables on to mono-rail cars. The cables became stuck so Employee climbed onto the mono-rail car to see why the cables were stuck. Employee slipped on the car and fell straining both knees. Employee did not violate any laws, policy, or procedures and is properly trained. Had on all required safety apparel and has a positive attitude towards safety.
Employee was walking back to the truck when employee slipped on ice and fell on back.
Injured was clearing a rock at the #4 Tailpiece and slipped on belt structure striking left shoulder.
Men were working on a M99 Mac Dump Truck removing spring pad belt from spring. EE was kneeling down on the right back end of the truck next to bindle hitch, when the truck started to shift to the right, slipping off blocks pushing towards ee shoulder.
A fully trained miner was injured when a rock fell from the top onto the slider tube. The slider tube then pushed the employee against the miner injuring neck and upper back. The injury was not a result of any compliance issues nor were any protective guards involved.
EE was injured when motor EE was operating along with another motor and 2 loaded flatcars became runways and collided with multiple manbus parked and being operated along track.
While installing wheel on locomotive employee pinched right hand resulting in laceration (requiring stitches) and broken middle finger tip.
Truck driver was moving a fuel hose by kicking it and suffered a broken right foot.
A fully trained miner was injured when a piece of rock fell from the top striking Rt. hand and Lt. Knee while tramming a motor. The injury was not a result of any compliance issues nor were any protective guards involved.
While adding Hydraulic and Transmission Oil to LD#998, the employee saw that the lube truck was leaking oil, and in the process of running over to push the E-Stop Button, employee strained right calf muscle.
While hanging miner cable and waterline, employee stepped on a rock and turned Right Ankle. EE stated that EE felt a pop in the heel/ankle area. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, machinery, or attitude.
Employee was flushing out the power washer water tank with a 2 inch water hose. EE was using bleach to clean the tank. When EE pulled the hose out of the tank, the pressure of the hose pushed EE back and caused EE to become unbalanced. EE fell approx. 3 feet from the top of the toolbox to the ground. EE fractured 2 ribs and pulled a groin muscle.
Employee was cutting down gob with the miner in #9 entry, when something came off the head striking EE in the right jaw, this caused a laceration that required 7 stitches.
Employee was dragging waterline down the HG5 beltline when ee stepped on a chunk of coal causing self to twist right ankle. This resulted in a sprain.
Employee was lying on left side shoveling and strained right shoulder
Employee stepped into a hole in the mine floor and twisted ankle. Employee finished shift but reported off under doctor's care on next scheduled shift.
A fully trained miner was taking ropes off of a supply car when EE sprained lower back. The accident was not the result of any compliance issues nor were any protective guards involved.
Employee was walking and slipped while handling hog panel at the face in the First East Headgate. Employee braced themself with right hand straining wrist.
Supervised Geo Temp employee working as a drill technician reported an eye irritation. Employee reported that the eye irritation was from dust.
Employee's foot slipped off rung of fixed ladder and employee lost balance. When landing on floor employee rolled employee's ankle. Sprain resulted. No coast, surgery required.
Employee stated that EE was climbing up the ladder on the hauler to check out a steering leak. The lower step swung underneath the bumper causing EE to pull self up the ladder. EE felt pain in the arm, neck, shoulder and back area. Went to doctor to get checked and missed work starting 2/21/17.
EE was getting off a motor grader by going down the ladder left foot remained on the ladder and ee stepped onto the moldboard when right foot slipped and ended up doing splits resulting in a pulled groin muscle in left leg.
Miner was bitten by a spider
On 2/19 employee was helping partner pull a jackleg out of the backfilled floor when EE felt a burning sensation in the right shoulder. When EE reported for the next shift on 2/20 EE reported the incident to supervisor and was then seen by the site medic & referred to the ER. X-rays were negative, restrictions were issued for muscle strain to right shoulder area.
Employee stated that while tramming the Fletcher walk through roof bolter, a T3 strap fell from the side of the roof bolter and struck left foot.
Coupling on rock dust fill hose separated and struck person in side of head.
Employee was struck by a dropped object from upper level.
Employee was shoveling salt in the screen plant. Employee stated employee felt pain in employee's neck. On 3/1/2017 employee went to employee's doctor and was put on restrictions and did not report to work.
Steam 3 chute plugged up, cleared chute with water hose, threw hose down to next level to clear pan under belt. Needed more hose to reach further and it was stuck on top plate. Pulled hose across my body to free it. Felt pain in left shoulder immediately. Continued working. Went off on April 2, 2018 to have surgery on shoulder.
Employee stated EE was manually scaling with a bar and EE's left wrist became painful and swollen. EE was sent to doctor locally and remained on full duty. EE's wrist became more swollen and was sent to a specialist on 02/24/2017 and the specialist placed EE on light duty. We could not accommodate light duty so EE is currently missing time as of 02/25/2017.
Employee was tramming scoop down entry, while turning into x-cut employee caught fingers between stop block and operators kitchen. Cause of the incident was lack of employee concentration. PPE was not a factor, mining equipment was a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.
Ee was blading in equipment, hooked a rock with the blade, and was jolted sideways, causing immediate back pain. Seen at ER and put off work for 3 days. On follow-up, off work was extended and physical therapy prescribed.
Operator was boarding a dozer and slipped and fell to the ground.
Employee smashed a thumb between a crib block and the body of a shuttle car causing a laceration and fracturing the tip of the thumb.
Individual was moving core boxes from one pallet to another when they caught their foot on one of the pallets and tripped over the pallet twisting their ankle.
A fully trained miner was building an Omega Block wall when a block fell catching left middle finger between the block and the top of the ladder EE was standing on causing a laceration to the finger. The accident was not a result of any compliance issues nor were any protective guards involved.
Injured appeared to be repositioning continuous miner when the ripper head caught the injured, pinching EE between the cutter head and coal rib. Incident occurred on 5 N. Mains Section in no. 4 entry, approx. 25' inby crosscut no. 45. Injured broke 7 ribs, fractured right shoulder blade and a bone in upper neck, and punctured a lung. Injured has several bruises on chest.
Employee was prying on a liner when it came loose and fell on EE's foot. After walking around, foot started to swell; EE was taken to the ER, and referred to a specialist. ONGOING.
EE was traveling on a rail mounted personnel carrier when the carrier came in contact with another rail mounted carrier on the main line track unexpectedly.
Employee was lowering the battery cover on a scoop when they caught their thumb between the battery cover and the battery resulting a laceration requiring stitches.
Contractor EE was traveling on a rail mounted carrier when the carrier came in contact with another rail mounted carrier on the main line track unexpectedly.
EE was walking towards the thermal dryer from the shop when another worker hit EE with the 1994 f-250 truck from behind hitting EE on right back and right elbow.
Injury first reported to Foreman on 3/29/17. Date of incident and time are approximate. Employee has burning sensation in left shoulder joint when pulling fuel hose from reel on lube truck to fuel 385 excavator. Employee is still experiencing pain. Possible overuse injury from pulling hose reels many times per day.
Employee was operating the tailgate side of the shear in E28 longwall mining towards the tail. At 108 shield, a rock flung off the cowl and struck employee on the right shoulder. Diagnosed with partially torn tendons in right shoulder on 3/3/17.
Employee was laying out belt structure for belt move when an air hose blew apart at connection and employee took off running causing strain to calf muscle.
Employee was coming to office to get ready for work. Passed out unconscious and hit head on the concrete slab. Was found by another employee unconscious and came too bleeding from back of head, severe headache, double vision. Was taken to hospital emergency room by Assistant Director.
Taking tilt cylinder pin out from eye where it attaches to head frame. Took bucket off, backed scoop up, had head frame tilted forward. Pin still in the cylinder eye and one side of bracket. Scoop was shut off. Coworker exiting scoop and heard the head frame hit the ground. Coworker went to front of scoop and saw IEs foot was under the head frame. IE had 3 fx'd bones in foot.
The employee was pulling on a breaker bar (ratchet) which disconnected from the socket causing the employee to slip and twist employee's left knee.
EE was installing roof bolts, EE swung drill head out,a hose caught one of the bolts, EE reached across drill head when piece of rock (2'x2'x2"")fell striking the right forearm against the boom, rock slid down & struck the hand. EE was taken to ER,received 3 sutures in forearm,3 in the hand. EE fractured the long bone in the right hand just before the knuckle of the pointer finger.
Employee struck their head against a roof bolt while dragging cribs into old works. Incident caused tingling sensation in employee's neck. Employee was transported to a local hospital, treated, and released. Reported off the next day.
EMPLOYEE WAS USING TORCH TO DEMO STEEL LINERS OUT OF CHUTE. ONE OF THE LINERS WAS UNKNOWINGLY LOOSE AND WHILE PRYING ON THE LINER THE EMPLOYEE HAD JUST FINISHED CUTTING, THE LOOSE LINER ON THE ADJACENT WALL TIPPED OVER SMASHING THE EMPLOYEE'S FINGER BETWEEN THE TWO.
On 2/14 A loader operator was climbing into loader and felt a pain/spasm in lower back. EE immediately began icing and taking OTC medication. On 2/28 ee requested to go to the clinic and was diagnosed with a sprained lumbar ligament and returned full duty. EE had a follow-up appt. On 3/10 and was prescribed physical therapy making it a reportable injury.
Employee was advancing a longwall shield when a rock struck the employee in the middle of the back.
Was removing plastic from underneath front end of F350 dump truck, lying on creeper and prying out on plastic. Got bad pain in right side of neck that radiated down right arm.
Employee was cleaning floor in wash bay and fell when coming down stairs
EE was jacking up 62 jeep on mainline near 2S switch EEs pushed jeep to get back on track when the jack bar hit EE on left side injuring left side ribs. 5 days later it was found EE's spleen was bleeding. EE didn't miss work until 2-20-17. It didn't involve job skills, compliance issues proficiency training attitude PPE protective devices No other part 50 criteria apply.
A fully trained miner was lacing cable slack on the boom of the loader when employee turned employee's head employee's hard hat hit the cable and felt pain in employee's neck. This was not a result of any compliance issues nor were any protective devices involved.
An employee slipped and fell on icy ground. Employee worked employee's scheduled shifts and full duty until March 5th when employee did not report to employee's scheduled shift.

Employee is an equipment operator, running the same equipment every day. EE was operating a loader and said it made back hurt.
Employee was driving a haul truck, hit a bump and hit their head on the roof. Employee said they were wearing the seatbelt. Driver had the seat adjusted too high, was possibly driving too fast and wasn't reading the road. Employee did not report the incident until 2/16/17 after a Dr's visit where they were given the diagnosis of cervical strain.
A maintenance worker was rebuilding a 12X14 pump and case. While working on the pump, it slid off the pallet and landed on top of foot. The employee was taken to the emergency room by a co-worker. X-rays showed no fractures found in the foot. The doctor did not prescribe any medications at this time. The employee was taken off work after the follow-up appointment.
Employee was leaving the drill using the main steps. As the employee reached the bottom step, the employee lost balance and fell forward landing on the left shoulder.
Employee walked out of bldg. with sample and slid on rock salt on the cement, falling on both knees. EE continued to work until 2/21/2017 when Dr. restricted EE to no work.

Employee reported EE was replacing sheave wheel on the 6X10 Diester screen when the wheel fell off the shaft and landed on right middle and ring fingers.
Fueler was driving fuel truck and drover over a windrow. This caused the steering wheel to jerk and pull EE's left arm. Fueler reported the incident and the EMT looked at EE's arm. EE returned to work. On 8/8/17 the employee informed the mine that EE has been taken off work and will be receiving surgery due to injuries from the incident.
EE saw co-worker at Screen 15 with the hood opened 3/4 of the way. The hood was blocked by screens stacked in front of it. In an attempt to lay the hood flat, EE crouched under the hood to lift/push screens out of the way and ee felt pain in lower back thereafter. Pain did not go away, EE out on injury from 3/15/17 and sought medical attention on 3/17/17.
Employee was walking along supply cars at the end of the section track and tripped on a piece of track rail causing employee to fall. When employee fell, struck right knee on the rail. No issues with equipment, PPE or training.
Employee opening #11 filter press, press had not competed draining and caustic (liquor) came out when the press was being backed up. Caustic (liquor) sprayed employee lower legs. Employee sustained caustic chemical burns to lower leg area.

A fully trained miner was moving a Z-Drill in the 14 East section. While doing so ee twisted right knee. The injury was not a result of any compliance issues nor were any protective guards involved.

EE was walking across floor and stepped on some flocculent that had been spilled and not noticed. Employee fractured a wrist.
Employee started to step down from a ladder The ladder shifted and pulled rivets from the inside of the ladder that did not appear to have been previous damage. The employee fell from the second step down to the mine floor and injured the right ankle. Employee worked the remainder of the shift with no issues but was later taken off work due to swelling.
I.E. ducked due to height of the roof as EE walked along the beltline rib. A corner of screen wire that had gapped down from the roof caught EE's ear causing 2 lacerations. (Height from floor to roof - 66"", from floor to screen wire - 59.5"", from screen wire to rib - 21""). Employee was taken to local physician where EE received 10 sutures.
Employee alleges employee was walking by the shop trailer and employee slipped on a belt that was covered by some snow causing employee to fall to the ground cracking 2 ribs.
A roof bolter was installing 8' bolts. Due to bolt length being greater than seam height these bolts must be bent prior to installation then straightened once inserted into the hole. While straightening the bolt, hand slipped off. The bolt sprung back and struck I.E. over the right eye causing a laceration of approximately 2"" that required sutures.
While walking into the guard shack, the door was flung open, hitting the employee in the left arm, causing employee to fall back, jarring the shoulder and neck.
While attempting to hook overhead crane up to scoop batteries, hook from crane rolled over pinching employees finger between crane hook, and battery lifting bar. This did not involve any compliance issues. Personal protective equipment was being used and this didn't involve job proficiency, machinery, or attitude.
While attempting to remove a bolt, the EE was operating 18 volt cordless drill and pressed the trigger with the intent of loosening the bolt but it was set for tightening instead. This movement jerked the wrist causing pain. On 2/11/17 EE sought medical attention and was diagnosed with a wrist fracture.
Employee was making an examination of the west main 4 belt when a coal rib rolled out causing the employee to jump back and slip and fall landing on right hand and wrist.
Dozer operator was pushing shot material. Fog conditions moved in and restricted the operators visibility but EE continued to push material. The operator became lost and drove over the high wall.

The employee stated was riding # 5 EMU down the track when a draw rock struck the employee on right shoulder, the piece of draw rock was 1/2""x2""x1' long. The employee sought medical attention on 8/30/2017 and was taken off work. This incident didn't become reportable until 8/30/2017. This incident will be a lost time incident.
The employee was shoveling rockdust into a bantam duster with back turned away from an oncoming examiner that was traveling toward the employee in low visibility. The examiner bumped the employee's leg and pushed it into the rockduster.
While picking up the roof bolter cable to hang, the operator of the roof bolter trammed the pinner forward, the pinner cable reel stuck causing a strain in employee's left shoulder area. Employee was taken off work on 04/28/2017 for surgery.

Employee was climbing steps on a Komatsu end loader and EE's footing slipped and EE fell.
Employee twisted knee when climbing down from excavator. When employee turned in FMLA/Disability paperwork on 5/31/17 for upcoming knee surgery, they had noted on the paperwork that the injury occurred at work. When employee requested time off in Feb for a Dr appointment for knee pain, they were asked it if happened at work and they responded ""No"".
Employee was attempting to remove ceiling tiles from a 8' ceiling in mining office. Employee did not have proper ladder for project and was working out of an office chair when the accident occurred. Employee twisted a knee and is awaiting an MRI on 02/13/2017. The x-ray's showed possible torn ligaments/tendons to be determined after MRI.
Employee was blocking up track switch with crib blocks. While maneuvering around the switch EE stepped on a crib block and turned ankle. After turning ankle, EE stumbled and struck knee on the crib block as EE fell.
IE had just pulled the continuous miner into the next place and reached down to grab the cutting loop rope to throw the cable over to the rib. IE had pulled the cable tight with the miner and while bending over to pick up the cable, felt pain in lower back. After follow up Dr.'s appointment the following day, employee had restrictions which prevented EE from working.
On 2/6/2017 employee was making repairs on a diesel man trip. Pushed up on a ratchet and felt left shoulder pop. Worked on full duty until 4/24/2017, when EE was scheduled for surgery.
EE & another EE was putting the trailer that contained the welder into the shop. The pickup that the trailer was hooked to died and would not start. EE got a floor jack to move the trailer on into the shop. The jack hit a crack between the shop floor and the landing the trailer dropped off the jack. Allowing the handle of the floor jack to come up and strike ee in the face.
Employee was pulling a plugged lance at Niro 2. The bleed-off valve was plugged, causing hot slurry to spray onto arm. Received 2nd degree burn on left forearm.
The employee was using a forklift to load pallets of cinder block into the scoop bucket. Unknown to the operator some blocks had come loose and had fallen in the roadway. As ee trammed the forklift ee ran over one of these cinder blocks and ee felt a pain in back from the jarring action. (Note: Received information that doctor had taken employee off work on 02/15/2017).
Employee alleged employee bent over and was pulling a 1 inch hose out of a supply job trailer and received a strain in employee's lower back.
The continuous miner ""CM"" on the unit was down and in need of mechanical repairs and the CM operator trammed outby the last open in preparations for the mechanic. EE was driving EE's maintenance ride to the CM to make these repairs and EE pulled through a check curtain striking the CM with EE's ride. EE began missing time on 2/7/17 for a bruised/sprained ankle.
Employee slipped near the #7 belt head rock box and twisted EE's left ankle. The employee was taken to Raleigh general hospital where an x-ray showed EE's ankle was not broken. The employee then had a follow up appointment on 2-8-17 where a second x-ray showed that the ankle was broken. Employee is currently off of work and EE's return to work is unknown at this time.
A fully trained miner was removing track ties from the 16 West track. While unclipping ties the miner felt pain in left arm. This was not a result of any compliance issues nor were any protective guards involved.

The EE was in the process of removing a hydraulic filter from drill 812 when EE injured a middle finger. EE first removed the bracket that secures the filter housing assembly. EE then held the filter housing assembly with left hand and began to remove four bolts located on top. When EE removed the last bolt, the filter housing dropped and crushed the left middle finger.
Employee was building a stopping wall and strained lower back.

Employee was cleaning up the walkway and picked up a spill board that had coal on it. While bending over to pick up the piece of steel, employee turned and felt a pain in the chest. Was taken off work by doctor beginning 2/6/2017.
On 2/2/2017, an employee was exiting a vehicle and twisted ankle. EE did not report the event until 2/8/2017.
Employee was stacking cut to size stone on a pallet. When employee set down a piece of stone on top of another employee had employee's finger underneath the stone, this caused two cuts on employee's finger that required three stitches.
Individual was walking through water approx. 10"" deep and slipped and tripped on a submerged airline in the walkway. This causing the individual to fall, hurting lower back and right knee.

Miner reported having a preexisting back injury prior to working for Big Creek. EE was lifting an air bag in the course of ordinary operations and reported a sharp back pain from lifting. There was nothing unusual about the conditions when the injury occurred. There was no damage to equipment nor injury to any other employee.

Miner was carrying a removed idler from a conveyor head section on the shop. EE dropped the idler and EE's left index finger got caught in the idler. The miner broke the last digit bone and also required stitches.
Employee was operating a bulldozer watching other employees put together a 24"" joint of pipe. EE exited the tractor and was walking toward the employees to assist in the task as they flagged the rubber tire operator to let the pipe down. When the rubber tire operator let the pipe down, the pipe shifted forward striking the employee in the lower leg causing EE to fall to the ground.

Employee stated that while dismounting facing toward the inside of the cab of the vehicle, pant leg got caught on the edge of a bracket. When the pant leg caught, employee lost grip on the handle and fell to the ground. Employee landed on right arm, resulting in a bad sprain and muscle contusion.
A millwright sustained a sprain to left ankle while traveling by foot in the Transfer Building. The miner states did not slip, trip, fall or step on an object just that the ankle rolled as miner was walking towards the stairs. Miner did not request medical treatment until March 1, 2017

Employee was entering mill elevator and door slammed shut on hand and injured one of the fingers on left hand that resulted in stitches.

As a scraper operator was running in the spoil cut area, was looking back at the apron and bowl and hit a large partially covered boulder. As operator made contact with boulder, the scraper bounced hard and the seat bottomed out causing operator to strain lower back.
Ee was open/closing air door, latch came down and hit hand, causing an open fracture to the left hand pinky finger. Ee was transported by light vehicle to Renown in Reno where surgery was required to pin the fracture. Ee unable to return to the mine site until after follow-up on 02/17/17.
The injured was sitting on the end of a shuttle car with feet on the ground when another employee, who was operating a three-wheeled personnel carrier, inadvertently struck the injured with the personnel carrier.
Employee was pulling a rope that raises and lowers the Carbis safety cage on load out for bulk trailers. When pulling the rope to raise the cage from the trailer, ee right shoulder made a popping noise and immediate pain in shoulder. EE went to a physician on 2-9-2017 and was given restricted duty. EE returned to work on next shift.

EE was walking between plant equipment,stepped on a rock that was under snow and material and twisted knee while catching themselves. Was seen by a physician 2/10/17 and placed on restricted duty and provided a knee brace until cleared by physician.
A process operator was digging a ditch with a pick axe and inadvertently struck self on top of right foot.
EE was walking in a crouched position, when a piece of rock struck the EE left ankle.
Employee was attempting to load wire mesh that was leaned against the coal rib, onto the Fairchild scoop when the mesh fell and restrained the employee between the mesh and the back of the scoop. This resulted in an abrasion to the abdomen.
Employee was carrying belt block up stairs in Feb. Employee was fine until May, when employee went to personal doctor, who said employee had a hernia stemming from carrying the blocks in Feb.
Employee descending stairs, and missed last stair. Employee was using 3 points of contact and caught self instead of falling. EE jarred right shoulder/bicep in the process. Torn right bicep diagnosed on 7/19/17.
EE and another employee were trying to get a side board off of a trailer. When the other employee was prying it from the tractor trailer it popped off and hit EE in the bottom right leg.
Employee was stepping down from a ladder to the main floor when employee twisted ankle.
Extension cord caught on truck was throwing off from truck, stepped off truck, landed on both feet, right foot tibia fracture.
Employee was using sledge hammer to break free plugged material from frozen flux bin. After, ee felt a pain and tingling sensation in lower abdomen. On 3/1/2017, ee was diagnosed with a hernia.
The bottom was laying sideways. The mantrip went into a dip. When the mantrip came out of dip, spare tire caught cable bolt tearing spare tire loose and it rolled over top of employee striking leg, chest, and face.
Employee was pulling floor pump pipe on C floor. Pipe became loose striking employee's left foot causing fracture. Restricted Duty.
Driver fell off truck, dismounting, while performing an annual inspection at scale house. Windy and gusts up to 45 - 50 MPH that day had a part in blowing door against the driver, causing EE to fall.
Got right foot caught in the bottom rung of fixed ladder while coming down, fell backwards and twisted right ankle.
EE bent over and back tightened and started to have spasm.
Sprained right ankle walking on edge of plywood decking on gravel surface; twisting ankle on loose gravel.
Employee was moving a rock from a haul road and strained back. Evaluated as first aid only on 2/2/17, and re-evaluated as restricted work duty on 2/7/17.
Inserting cable bolts into drill holes on day shift working from a JLG man basket. This required the employee to reach away from EE's body while pushing the bolt into the drill hole. After installing 15 bolts, employee back felt sore. EE continued to install bolts with support of other employee. At lunch felt pain in back, was seen by doc, diagnosed with lumbar sprain/strain.
Employees were descaling spool using a hand maul. The injured employee was bending forward looking at the pipe spool as the 2nd employee was swing the maul the maul struck the pipe spool then bounced off the pipe spool striking the injured employee on the left side of the face.
EE was on elevated platform erecting equipment; reached back with left arm (fully extended) to guide/pull scissor lift to platform; felt a ""pop"" and left arm went temporarily numb with tingling in fingertips of 4th and 5th fingers; took a break but declined offer to come down; attempted to ""work out"" discomfort; later, left arm ""went out"" and miner requested assistance.
Employee was installing screen on the setup and as EE was reaching down to grab the screen, EE slipped causing hand to be cut by the screen.
3 employees were involved, they were replacing the cutting edge on the loader. they had lower the bucket on the cutting edge there were bolts bound up, when they were to free up the edge EE and IE had there ends on the edge of the cutting edge causing to bruise there left hand equipment involved was. 972M Loader
Was lifting old top box from roof and lacerated web of left hand between ring and pinky finger, had gloves on.
Employee was standing on two 4D batteries, not performing work, as employee stepped off them employee stepped down onto a water hose rolling employee's right ankle.
EE walked a few feet to pick up a caulking gun laying on top of a screw housing (EE did not have to bend over) and when EE turned to walk back the other direction EE felt a pain in the middle back. EE was sent for medical evaluation, diagnosed with a back strain and released to work with restrictions.
Employee was leaving the cab of the dredge when employee realized employee had forgotten a paper when employee went inside employee held the door open with employee's left hand when employee's hand slipped going in between the door and the door frame. Door caught Left middle finger
While task with installing steel door frames in the Jumbo covers on a conveyor belt-roadway, two employees attempted to move the door frame manually to allow a vehicle to pass. The injured employee stated that while the employees attempted to lift the door frame, employee felt a sharp pain in employee's lower back
The roof bolting machine was moving to another entry, there was approximately 15 feet of cable slack behind the machine. The cable reel retracted the slack and the machine began to tram forward, the miner was struck by the cable hanging from the mine roof when the cable was pulled down from the hangers. After medical evaluation, the miner has been placed on restricted duty.
Employee strained back while operating rubber tire dozer on 2/26/17. Employee saw doctor on 4/24/19 for back pain that was attributed to 2017 incident and was referred to specialist and given restrictions.
Employee was assisting other employees in placing a shaft into a mechanical separator. The shaft shifted and caught the employee's right thumb between the shaft and separator wall. The incident resulted in a laceration to the right thumb.
The employee was assisting another employee with jack leg drilling, placed hand over hole to prevent blow back, caught finger between jack leg and rock.
Un-banding switch ties. Cut one band and was repositioning self to be clear for the next one when band snapped prematurely, causing ties to roll onto left foot.
Employee was lifting a heavy metal plate and experienced pain in the right side of lower back and down right leg.
EE went to chock truck at wash rack. EE chocked driver's side wheels and stepped down approximately 1 ft off the rack to the concrete below and rolled right ankle. EE sustained an sprain - issued restrictions.

The injured was drilling the hole for cable bolt when the drill steel fell back out the hole striking employee's finger.
Employee bent over to lift a chain from the ground when ee felt a pull on right side-groin area. EE was diagnosed with a groin sprain.
EE was operating a remote box for a miner when a piece of rock struck right hand causing a small laceration. On 3/1/2017 during a follow-up visit to Dr. EE was placed on restricted duty to prevent potential infection.
EE was cleaning in pack house basement. While EE was dumping material EE tripped causing a strain to EE's left groin area.
Employee was attempting to tram the #753 tub out of the track spur when the tub derailed. EE jacked the tub up until it was above the ball of the track rail. When EE got it high enough EE was going to attempt to shove it off of the jack to make it land back on the rail. In the process of shoving the tub over it landed the wrong way and landed on the employee's foot.
Two maintenance persons were tossing a piece of irregular shaped scrap cast iron pipe into a scrap metal dumpster. Piece caught edge of dumpster and spun and pinched two fingers between pipe and dumpster
EE was bolting top when the drill steel bent and broke hitting EE in the right arm.
EE was climbing into screen #701 to replace skirt rubber that deflects rock at the end of screen. While entering the screen EE had to push existing skirt. While pushing EE heard a ""pop"" in their back but did not feel pain so continued & completed the task. In the next morning while getting up EE's back was sore & EE notified supervisor (mill foreman).
The employee was assisting the Lead Man with the workplace exam and stepped through a doorway from the MCC to the mixing room for thickening tank flocculent. There was a wet spot on the floor from a roof leak caused by a storm. Employee slipped and sprained left ankle.
Employee was driving haul unit loaded with slurry up a ramp. Tires began to spin and lose traction on the slick ground. Unit began to slide backward then tip back onto its end gate causing the unit to sharply jolt backward causing pain in employees neck and back.
Two employees were pulling electrical cable when the rope broke. One employee lost their footing and fell backwards on to their back side.
EE was operating a dozer while backing up and drove up onto a large chunk of slate. The dozer slid off the chunk and jostled the employee in the cab of the dozer. The employee felt pain between shoulder blades directly after.
Slipped over pipe ee was working on and through out hand to break fall
EE WAS PULLING A ROPE ON A TROLLEY BEAM WHEN THE ROPE BROKE CAUSING EE TO TWIST RIGHT ANKLE. EE WAS PLACED ON RESTRICTED DUTY WHILE TESTING WAS PERFORMED, DIAGNOSED WITH A FRACTURE OF RIGHT ANKLE AND EE REMAINS ON RESTRICTED DUTY.
Miner was walking down provided stairs and using handrails in railcar preparation area and upon stepping off of last step to the ground rolled right ankle. Miner fell to the ground and continued work for several more minutes and then reported injury.
Employee was coming from the cement silos heading to the car shaker walking across the railroad tracks when EE twisted the left knee. EE was taken to the company clinic where was seen by a doctor and diagnosed with Sprain of unspecified site of left knee and prescribed Physical therapy. Weather conditions and visibility were good at the time of the event.
Carried a bundle of conduit, as employee twisted to place it down on the floor. Employee's lower back popped.
Employee was changing wire rope on overhead crane when fingers were pinched between the drum and the wire rope.
EE was raising the gantry when the gantry fell and pulled the EE's arm.
EMPLOYEE STEPPED DOWN FROM PLATFORM & TWISTED ANKLE.
Two employees were changing a bearing on a tail pulley. Employee was holding the bearing with both hands while the other employee was tapping on the bearing trying to loosen it from the shaft. The tail pulley assembly shifted/fell 2-3 inches and crushed the tip of employee's ring finger between the bearing and the frame.
Red line case. On 2/16/2017 employee got in and out of the bucket of the HV bucket truck 5-6 times. Employee did not feel bad at that time but reported the injury on 2/22 after knee got worse.
Two Maintenance mechanics were using an overhead crane to lower a mill down onto some metal cribbing for rebuild, when the lifting bolts failed and the mill fell approximately two feet to the Shop floor. The mill struck one of the mechanic's left forearm, resulting in swollen and bruised forearm.
While removing rocks from a crusher 1/30/2017 and welding in a crusher the next day employee reported a strain to right knee. On 2/16 ee was evaluated and the doctor prescribed Physical Therapy making the injury reportable. EE continues to work full duty.
A maintenance employee supporting production to ball Finish Mill #6 was the flagman situated at the top of the mill giving instructions to the crane operator to place the grinding media drums in the mill door. EE placed hand between the drum and the device used to carry them, the drum shifted and smashed left middle finger.
Employee was walking down steps of Plant 1 Operator's Shack, and when employee stepped off last step they landed on an uneven spot and twisted the left knee. They went to OMP and is on restricted duty.
Employee was climbing down the stairs on the loader and slipped on the mud on a stair and caught self while falling backwards and felt a pop in left shoulder.
Employee stepped down after replacing a hose and twisted employee's knee (employee felt a pop). The employee sought medical treatment 02/16/2017 and was given restricted duty.
Improper lifting procedure to recover fallen handle held stone breaker. Pulled muscle in hip. 2-day lifting restrictions of 15 pounds.
When employee was picking up a rock to move it to the conveyor belt. Employee moved their upper body while their foot stayed in place which caused their knee to sprain.
They unbolted the rotex and changed the bottom 6 screens on both sides of the rotex. Employee then bolted up the rotex and then came down to the control room. When employee got out of the chair they could feel a sharp pain. Employee thought it was nothing at the time and would go away. When employee woke up on 2/16/17 they had more pain.
Injured was using an air chisel to remove ash ring from #2 kiln when a piece of ash fell and struck shoulder/arm. Injured did not request medical treatment until 2/16/2017.
Employee was pushing on the cables of a hoist, the left ring finger and pinky finger were pulled into the pulley of the hoist.
Employee bumped an unsecured pipe stand (4"" wide by 8' tall) while closing a valve. The pipe stand fell over striking another mechanic on the hard hat and knocking that EE to the ground. The next day EE complained of pain in knee. Was given restrictions.
Walking to the water jack, the employee's right leg slipped out from under EE and caught self by planting left leg. Emp was seen by company nurse on 2/13 given ibuprofen and ice packs. On 2/16 company nurse re-evaluated emp placed employee on Restricted duty due to the swelling and complaint of pain.
EMPLOYEE WAS LIFTING A HEAVY POWER CORD AND WHILE BACKING UP, EE TRIPPED ON CORD BEHIND EE AND FELL BACKWARD ONTO FLOOR.
EE was cleaning grease on the floor of Ball Mill 1, slipped and struck guard
Employee was struck by a small piece of rock on the right foot. Employee suffered a contusion to the top of foot. Employee continued to work until 2/14 when employee went to the ER with a small swollen area. The employee is a diabetic and as a precaution the ER Doctor recommended restricted duty for 3 days. Employee returned to normal duties.
Employee was moving a piece of refractory on a conveyor when it rolled and pinched employee's hand between the refractory and the frame work of the conveyor. When employee's hand was pinched, it caused a laceration that resulted in 7 stitches on employee's ring finger of employee's left hand.
EE was attempting to remove some draw rock off top of cont. miner. EE grabbed a piece of rock measuring 30""L x 24""W x 5"" thick & pulled it toward body. When rock reached edge of machine EE lost grip, rock dropped down & caught front part of ring finger on rt hand against edge of machine severing most of the flesh toward the bone. EE was working in no. 6 entry w/2 co-workers.
Employee was scoping a job in a manlift. Employee was reaching out and up to simulate what needed to be done (not lifting anything) when EE felt pain in back. The pain did not continue and EE completed the job. After leaving work and over the weekend the pain progressed and reported pain when arrived to work on Monday 2/13.
Employee stated EE was standing inside the bucket of the scoop and loading supplies onto the roof bolter. Emptied a wooden pallet and turned to throw it out of the scoop bucket. As EE turned EE's feet slipped and EE fell over the side of the scoop bucket resulting in sprains to multiple areas of EE's body and restricted work days.
Employee was holding angle for welder to tack up in hopper. From 11:30PM to 6AM. Employee was closing eyes as welder was welding. Employee went home and woke up with burning eyes.
Walking backwards while using a hose to clean a discharge ditch employee lost balance falling backwards (on the same level). Employee sprained left hand/arm trying to break fall.
The injured smashed finger between the belt rail and the top belt roller while removing the roller for a belt splice.
Employee stated was servicing a coal loader. Squatted down onto knees to change the engine air filter. When employee did, felt a pop in right knee resulting in pain in right knee. Went to doctor to get checked out. Was put on restricted work duty.
EE working on 992C Loader in shop. EE removed the Shut-Off Solenoid from RR of loader; it dropped to the floor. EE descended loader to pick up the part. EE slightly bent knees & bent over at waist to pick up part when ee felt a pop & pain right knee; ee then fell to the floor holding the knee. EE suffered a Transient Patella Dislocation (small knee cap dislocation).
EMPLOYEE AGGRAVATED PRE-EXISTING ELBOW INJURY WHILE USING A CHAIN BLOCK.
EE had just completed swapping thrust roller on kiln. The EE removed employee's safety glasses to wipe employee's eyes. Employee believes a piece of debris came off EE's glove when EE rubbed EE's eyes. EE worked rest of day. Returning to work the following day, EE was sent for medical assistance due to redness and discomfort. Medical personnel swabbed EE's eye to removed object.
EE was using a hammer to remove a bolt. EE had left hand holding onto bolt. EE hit left thumb with hammer.
Employee was lifting a large stone approximately 40 pounds and strained their back
Miner bent over to pick up a piece of flag stone and strained back.
The employee was walking to the working station and tripped and fell over a box, landing on left leg on the side of the knee.
EE felt knee pop when exiting equipment.
Employee was changing tires on a 993K Loader. While installing lock ring on the wheel it slipped striking employee in the head.
Employee was replacing a ballast on a light pole; and as EE was holding the ballast EE reached down to get a battery drill. Ballast slipped out of left hand and struck right hand cutting right hand requiring sutures.
Employee was putting a track pin back in the Excavator, a piece of steel broke off sledge hammer or pin and embedded in right hand between first finger and thumb.
Employee was grinding on a piece of metal with a grinder when a small shard of metal flew up under face shield and safety glasses and became embedded in right eye.
Employee was changing seat compressor on a front end loader, the porta-power jack slipped causing the blocking to come loose allowing the seat to fall on employee's hand smashing the middle finger of the employee's left hand resulting in an open fracture.
Employee slipped on wet, partially icy area and twisted the right knee. Did not fall. Was referred for physical therapy
A large piece of frozen sand caught on half on the belt and half off causing plugging. Employee removed the chunk of ice off the belt and placed on the ground. Employee did not feel any pain, but yet felt something that wasn't right in the groin area.
EE was cleaning out a pilot hole with a PVC blowpipe so EE could wire saw when the pipe separated and struck EE on the right forearm causing a contusion.
Employee was installing an intake boot when they felt a pain in right elbow.
Employee was walking quickly up two flights of stairs on Station 4 when employee felt tension in the lower left back. After making it back down the stairs to a pick-up, the tension turned into extreme pain.
After performing cleaning operations the employee slipped on and fell onto a horizontal pipe striking mid-section on the pipe. Employee reported the injury to management on the 10th of February. Employee was taken to clinic and diagnosed with bruised ribs. Employee was placed on light duty for a week and given prescription medication to aid in the recovery.
EE was rotating a gearbox with a hoist. When the gearbox was lifted it started to spin. EE reacted by putting left hand out to stop the spinning and EE caught hand between the gearbox and wall. Minor bruising. The next morning EE complained of pain in L knee. EE had x rays. Everything is fine, but placed on light duty for a few weeks.
Employee was exiting a Skid Steer Loader. When EE rose up to get out of the seat, EE felt pain in lower back. The pain got progressively worse over the next few days. Requested medical attention on 2/6/17.
Employee stated was operating the RBM on the #1 Section, and as employee was walking through the center on the roof bolt machine, employee struck head against the mine roof. This resulted in a neck strain and restricted work days.
Employee strained left knee when dismounting ladder. When ee stepped down ee skipped the last rung and stepped down onto left knee and felt pain.
Crew was dipping center pin of shovel in liquid nitrogen when it started to bubble out of the tank coming into contact with employee's left hand.
Employee picked up a 35# piece of steel with the left hand it hit a beam above and dropped on to the 2nd finger of the right hand. Splinted.
Employee states had been working all day in a wheel loader while the seat's air system was not working and developed a strain in lower back.
Employee slipped on some ice and fell, hurting left wrist.
Dredge operator was cleaning roots out of suction line and was closing the cover to the clean-out when a pry bar, employee was using to close the cover, slipped hitting employee in the left eye lid. Injury resulted in stitches only to the eye lid itself.
Employee was assisting in unloading water props from a trailer. A second employee moved one end of the prop causing the opposite end of the prop to smash the injured employee's right middle finger. The injury required stitches.
Haul truck had been towed to truck shop for repairs. Bed was raised using the hydraulic line from tow truck. While disconnecting the quick coupler to the hoist cylinder, the pressure in the hose caused the line to snap back striking EE in face. EE sustained lacerations and broken tooth.
EE was performing various tasks, welding and grinding. EE was not sure of the exact time or source but eye became irritated during lunch break and assumed happened prior.
Injured employee was flying a drone for end of month inventory activities. While landing the drone was blown off course and employee tried to catch it by one of the landing legs when employee made contact with one of the propellers causing cuts to two fingers on employee's right hand that required butterfly stitches to close.
Employee stated that EE was using a cut off wheel to cut off a section of an exhaust pipe. The wheel broke causing a piece of the wheel to fly off and strike the employee's chin. No lost time incurred but reportable due to requiring sutures.
Employee was cutting plastic ty-wraps off of a hose. The knife ee was using cut through the ty-wrap unexpectedly, and cut the back of left thumb. Sutures were needed to close the laceration.
Employee was adjusting a work light on a service truck. As employee dismounted the rear of the truck, employee stepped on a rock. When employee let go of the rear of the truck, employee's left ankle rolled.
Employee was crossing over the Ref belt on a catwalk to get to the classifying sump on B side. While employee was traveling over employee's hard hat came into contact with the discharge pipe of the spiral ref screen causing employee to chip a tooth.
Employee was dressing up a piece of metal when the hand held grinder kicked back and struck employee in the lower lip requiring 7 stitches.
While working on a manlift, dismounted onto the ladder misjudged the steps fell on right side shoulder. Initially said it was ok, shoulder continued to be sore, started physical therapy on 3/10, did not inform the company of this activity until 3/24. The diagnosis, strained separation in the shoulder, the employee has not missed any scheduled shifts.
Employee was installing the top row of blocks on a permanent ventilation stopping and got dirt in eyes. Employee went to a local treatment facility and was prescribed anti-inflammatory drops.
Employee was removing a rock from top of miner and lacerated middle finger on right hand. Required stitches.
Putting conveyor chain back on miner in St Leo rehab area, injured held pin while another miner drove it in with a sledge hammer. Hammer glanced off chain and struck injured in mouth, resulting in a small laceration on lip and chipping right upper incisor. No problems with PPE, training, mining methods or compliance issues.
Employee was cutting out a rock box. Metal from the rock box struck the employee on the back of the right hand. This caused a tearing laceration that required 3 sutures.
Employee was using a pry bar to unseat and slide out a tram motor. Motor moved, pry bar slipped, and employee's movement carried hands into the frame of the continuous miner frame. Left Middle Finger was pinched between pry bar and miner frame. 2mm was trimmed off the finger tip, 8 sutures were used to close the wound.
EE was removing a steel jig with another employee and asked helper to release the end helper was holding. The injured employee (who was only using one hand) lost grip and dropped the jig. As the jig hit the ground and rolled, it struck EE in the mouth knocking out one tooth and cracking two others. EE was medically evaluated, cleared and sent to a dentist for dental repairs.
Employee was extending roughneck slide while tripping out drill rods and pinched finger in between the slide and stop bolt.
Checking haul truck. After cranking truck, employee got out of the cab to go down the steps to the ground. When employee stepped on the first step a pain went through employee's hip to employee's back.
Miner reached behind the seat and cut right hand on a piece of glass.
Employee was on the dredge coming back into the wheel house from checking the bilge pumps. When employee went to raise the seat arm with right hand on the arm and left hand on the door jam. The wind blew the door shut on employee's left hand.
An operator was operating a Cat 992G Front End Loader feeding coal into the hopper. The operator ran over a chunk of coal that contained petrified wood in it jarring their lower back. The operator started physical therapy on 4/26/17 and will have 6 sessions making this injury reportable to MSHA.
Installing 3' rib bolt with 3' channel in upper rib to secure metal rib screen and bolt head torqued off. Bolt plate fell/sprung off screen (void between screen and rib) and struck miner in the left jaw, resulting in a laceration that required 5 sutures. Miner returned to work the next shift.
I.E. was hanging a vent tube up after it had been knocked down. A piece of rock measuring 4"" X 5"" X 1"" thick hit EE in the mouth causing EE to have 4 stitches in the upper lip.
Dyno Nobel crew was feeding loading hose through outbound hose sheave assembly. EE was standing on truck using 3 points of contact. EE's right hand was on hose boom carousel. Hose boom moved pinching EE's middle finger right hand between hose boom guide wheel and hose boom carousel. EE received 7 stitches.
EE was attempting to put air into the left front tire of the Genie forklift when the inside wall of the tire blew out. EE stumbled backwards and fell over a stack of crib blocks thus injuring lower back. X-ray were negative, EE was given a prescription for back spasm.
REMOVING HOSE FROM DUST COLLECTOR AND TWISTED LOWER BACK.
Employee was hammering down poly deck screens. EE struck the middle finger of the hand holding down the material, fracturing the tip.
A miner was injured while striking a chisel with a hammer. A piece of rust/metal went under the injured miners safety glasses frame and contacted the left eye. Employee was wearing prescription safety glasses when the incident occurred. Prescription eye drops were prescribed because of the scratch on the left eye.
Employee was helping welders weld on the shovel dipper. On 2/23, employee reported to supervisor that eyes were hurting. Employee was taken to the ER and given eye drops for welder's eye burn. It wasn't till 2/27 that it was learned that the eye drops were prescription.
Employee was repairing a piece of plant equipment and, while using a cutting torch to remove an overhead bolt, the wind blew a piece of hot metal into left ear causing an inner ear burn.
An employee was operating a Broderson IC-80 crane attempting to lift part of a crusher assembly when the crane tipped over landing on top of a haul truck box. The operator then unbuckled the seat belt to exit the crane and injured right shoulder.
EE stepped down 2 feet to lower level at reclaimer and felt a sharp pain in back. EE was taken to clinic and received medications for pain.
The employee tripped over a pallet and fell. Employee tore the Achilles tendon.
Employee was hanging 4"" pipe when ee slipped on the trailer and went to catch self ee fractured a bone in left hand.
Employee was installing a 3' chuck bolt in a flyboard and as the employee was straightening the bolt employee accidentally hit the rotation on the machine twisting left wrist. Employee was seen by a doctor and it was determined employee had a hairline fracture in the wrist. Employee was released back to work.
Chain fall was hooked to liner that had pad eye welded to it. The pad eye broke loose and liner fell and hit EE in the right thigh. EE received a laceration on right thigh.
After Jacking up a rail flat car to re-rail. 2 employees began to push car back on the track. 1 employee slipped causing both employees to fall & in the process the employee who slipped, boot contacted the other employee in the upper lip causing a laceration. Seen site medic & referred to ER, sutures were required, full duty release, no days lost, making it a medical reportable.
Contract employee was struck in the mouth when a chain that was wrapped around a beam slipped off from the top as they put tension to it. The piece of chain broke a tooth. Cause of this injury was employee awareness and positioning. Compliance with rules/regs, PPE, mining equip/systems not a factor. Mine proficiency may be a factor.
While getting supplies out of toolbox the lid shut on employee's left hand. This caused a laceration. Sutures were required to close the wound.
The employee was uprighting a manlift basket, lost footing, fell and struck left ear on the side of the basket.
The Miner Mechanic was loading a traffic cone onto holder rod on the back of a Terex water truck and dropped the cone on finger creating a pinch point to the gloved left index finger. It created laceration. Two stitches and tetanus given.
GMS employee was bolting and the drill steel fell out and caught employee's hand between drill steel and drill handle causing injury to hand.
An employee was walking down a stair case and asked an employee on the catwalk above to hand them their radio. The employee on the catwalk attempted to lower the radio via the mic cord. The employee on the catwalk let go of the radio causing the mic cord to hang up on the hand rail. This caused the trajectory of the radio to alter and strike the employee in the mouth.
Using a bar to dislodge a boulder and caught finger between handrail and bar
EE got a foreign object in eye while welding. Nothing was imbedded in eye, however, EE was given a prescription eye medication to prevent infection.
Employee was lining up a wear plate on a conveyor chute. As ee went to set it down, ee pinched right middle finger between the wear plate & inner lip of the chute. The employee sought treatment & was diagnosed with a fracture as well as a laceration in which ee received stitches.
While using an angle grinder the grinder kicked back hard enough to come loose from the operators left hand and the cut off wheel nicked left thumb as it passed. This resulted in a minor cut between the first and second knuckles of the left thumb which was repaired with 3 stitches. The operator returned to work and finished shift with no restrictions from the doctor.
EE was cleaning 2nd hearth w/air lance. Ball valve connected to air lance didn't have secured handle. EE used handle to open valve then put handle in pocket to prevent from falling off while cleaning. When finished EE tried holding air lance w/ Lft hand while using Rt hand to get handle out of pocket. EE lost grip of air lance & sustained a laceration to Rt leg. Stitches applied.
Employee was removing putlog pipe on 3rd floor. Employee was pulling on one side. When beam on opposite side of vessel came off refractory on port, the pipe came up and struck employee in right front tooth and chipped it.
EE was walk down stairs from 10th to 9th floor. When stepping onto the landing of 9th floor EE ""rolled"" ankle because of uneven grating.
Cutting metal - piece got in employee's eye. Doctor picked out 2 small pieces out of employee's eye. Came back to work on Monday.
Employee got down off the dozer at the edge of the clean coal pile and was walking toward the office, employee stepped in a rut and felt pain in the right knee. Employee had surgery to repair the knee on May 17, 2017.
Technician was removing parking brake assembly from excavator final drive. Tech used a hydraulic press to release parking brake. When tech went to remove snap ring retaining brake piston, piston blew out of housing, piston contacted tech nose and injury resulted in fracture and stitches.
On 2/17/17 while working on the outside of the Process Elevator the EE was pulling a bearing off from the outside while another employee had cut the shaft with an acetylene torch that the bearing was connected to. EE picked up the bearing by the shaft causing burns on EE's hand.
EE said slipped off the track when dismounting dragline. Later in the day EE went to Dr. Dr said no damage but has pre-existing conditions. Sent back to work. Later EE's back hurt. Returned to Dr & did physical therapy but no restricted duty or lost days. Received an MRI on 5/16/17. During MSHA inspection, Inspector & MSHA Supervisor said need accident report to start on 5/16/17.
Employee was changing bit sleeves on a continuous miner drum. A piece of metal from the hammer hit EE in the left forearm. EE thought it was just a small cut but later found out the small chip was in there. It was not reported until 2/28/17 when notified from Dr.
The 2 employees were attempting to reposition the blow bars in the horizonal crusher, while moving the blow bars it moved and struck the employees little finger on the left hand.
A continuous miner operator was injured while operating the continuous miner, when a rock fell from the mine roof and striking small left toe on the side of the foot resulting in a small toe fracture.
Load of angle iron was being dropped in a new location when the load slipped and fell. One piece of angle iron bounced and hit EE in the lower leg. EE had a laceration with stitches.
WHILE USING AN AIR ARC CUTTING TORCH DURING A WELDING OPERATION, WORKER SUSTAINED A BURN IN EAR CANAL WHEN AN AIRBORNE FRAGMENT OR SPARK FLEW INTO EAR. EE'S HEARING BECAME MUFFLED AND WAS SEEN BY AN EAR SPECIALIST WHO DETERMINED THAT AFTER TIME AND PRESCRIPTION MEDICATION, HEARING SHOULD RETURN TO NORMAL.
After working on an engine, crawled out from under truck raised up and hit water cannon mounted on back of water tank with head, causing a laceration
Employee had just adjusted impeller on a pump and caught finger between belt and sheave causing a laceration to tip of middle finger on left hand. Received stitches to tip of finger.
EE was operating continuous miner, when a hydraulic hose blew under the tail of the CM. Hydraulic oil sprayed EE in the face and oil came in under safety glasses getting oil in right eye. EE was administered first aid using eye wash. EE was transported to medical facility, eye was washed out and antibiotic eye drops were given to EE for treatment.
Attempting to remove screen decks from MK0601. Employee slid the screen along the deck in employee's (gloved) hands and the screen dropped into place pinning three of employee's finger causing a crush/laceration resulting in sutures (9).
While attempting to reposition a small section of chute liner, the liner came to a stop and the mechanics gloved hand slid along the bottom edge. The liner had been worn down to a sharp edge and cut through the employees glove and hand resulting in a 2"" laceration. The employee received stitches to repair the laceration.
Employee was changing a take-up pulley bearing. Employee attempted to remove the sleeve and rear race by sliding it off the shaft. The sleeve got caught on the burrs at the end of the shaft. Employee's finger got caught between the race and the wing pulley bushing. This resulted in the finger tip being broken and a laceration.
Employee was driving a stinger and ran over a crib block. The steering wheel spun and the steering knob struck the right hand. This resulted in a fracture to the 5th(pinkie)finger.
While cleaning frozen mud off of the gate of the diverter at the secondary screen the employee struck left hand against the door of the diverter.
An excavator over heated and ee turned the radiator cap and the steam was released and burnt left hand and forearm.
While grinding a weld off the bed of the dump truck utilizing leather gloves, repositioned ee to a squatting position while grinding in left hand requiring constant hand pressure, lost balance and caught himself with right hand on the deck of the dump truck. The grinder struck the floor and moved striking the top of right hand
Supervisor was helping to move a wooden light pole and left thumb pinched as second employee on opposite end of pole moved the end.
Employee received a burn to left eye from the welding slag while sitting beside a coworker. Received a prescription.
EE was attempting to cleanup a floc spill on the floor at the Prep Plant when EE slipped and fell and strained lower back. EE has continued to work but started receiving physical therapy on 02/23/2017. This is being reported as a Medical Treatment accident.
Improper use of knife to remove insulation from wire-out finger & required 4 stitches.
Employee tried to remove drill bit from a hydraulic drill. EE accidentally hit the trigger causing the bit to go through the glove and cut EE's thumb.
Worker had replaced dull utility knife blade with new one. Secured conveyor belt was being cut when towards the last portion of the conveyor belt the knife slipped and cut right thumb on right hand.
Employee was handling steel plates when one slipped an fell on their hand. Injuring the left hand index finger.
Employee was cleaning up at the end of the shift. Employee had just rubbed their eye and felt a irritation shortly after. The employee requested to see a doctor during the next shift. A small metal filing and rust was removed from the eye. Drops were prescribed.
Miner suffered a Left knee injury while descending a stairway. Did not slip or fall, and was using the hand rails when injury occurred.
Employee was leaving site for the day. Employee stepped out of the transport van, and as employee hit the ground felt right knee twist and pop.
A miner was using a pry bar to remove under grates in order to clean material build up from clinker cosma system. Pry bar slipped and the miner's left fifth didget finger was smashed between the pry bar and inspection door which resulted in a hand injury requiring two stitches to close wound.
While driving an articulating truck at the quarry, something fell into EE's left eye.
While installing a sweat on hub on the loadout belt conveyor drive, the hub stopped moving on the gearbox shaft 1/8"" shy of its desired location. The victim was struck by a glancing blow of a 4 lb. hammer in the mouth. The hammer struck the hub before striking victim in mouth.
Miner was assisting in attaching guards to conveyor. Used a pocket knife to cut away zip ties attached to conveyor and hand slipped, lacerating the finger of left hand. Transported to medical clinic and received five stitches. Returned to work the same day without work restrictions.
Employee was discarding old troughing roller into metal waste bin when it slipped out of employee's grip and pinched left 4th finger between the roller and the bin. Employee received stitches and returned to work on full duty
While removing the metal skirt board from the conveyor, employee's left pinky finger was caught between the skirt board in hand and chute. Employee was wearing glove for safety. Cut was immediately attended to in Emergency Room and received 4 stitches and antibiotics. No lost time days.
Employee was reinstalling a shield on the continuous miner and smashed finger resulting in an avulsion.
Using an air gun to put a bolt in, employee was holding bolt in place and by accident hit the trigger on the air gun .... spinning the bolt and cutting finger
Employee was grinding on a metal hopper. Wearing the proper PPE (safety glass and full face shield) employee removed the face shield and a piece of metal shaving fell under safety glasses in right eye.
Miner was checking the proper sequence of rail cars. The miner did not realize that ee had traveled past the end of fall protection lifeline. This stopped the miner and caused body to turn and fall onto the side of the rail car. Miner did not miss work but was sore so went to doctor on day off 2/15/17. X-rays showed cracked ribs. No other medical treatment was given.
EE was removing a bushing. A chisel was used to create a gap between the cross shaft and bushing. EE pulled the bushing (chisel still wedged in assembly) off the cross shaft. EE tossed the bushing 4'6"" to the side. The chisel dislodged from bushing & projected upward 7'6"" at a 45 degree angle back towards EE. The chisel struck EE in the lip under the right nostril.
Operator was climbing up the stairs of the loader to check out some recent damage to the machine and when the employee reached up to feel the metal on the hood, there was a sharp piece that had resulted from a recent fender bender that cut employee's right hand on the dorsal surface. (The machine was a Komatsu a WD-500.
Employee was picking up trash when a small piece of draw rock fell striking employee's right forearm resulting in a small laceration.
Laying on the shaker. Lift a piece of belt above head and twisted wrong felt burning and pain down lower back and legs
Contractor EE & co-worker were on the surface working on a rock dusting machine. After the repairs were completed, the IE was standing in front of the machine when the operator started the machine for testing. The IE started to step back & lost balance. IE reached out to grab something & got finger into an unguarded location resulting in cut to tip of finger.
This injury is stemming from a first aid injury that occurred 9/10/14 and was reported at that time but no further medical treatment was sought. This was again reported 02/10/17 with the employee stating it was due the the injury in 2014. Original first aid was due to back strain as a roving mechanic at that time. *EE was shoveling on 2/10/17 per call.
Employee stated was using a battery powered leaf blower to blow dust out of #3 D-Box and felt a small piece of dust or unknown object enter right corner of right eye. The employee went to optometrist after work and was prescribed prescription eye drops. This incident is being reported as a medical reportable due to the prescription eye drops.
Employee alleges EE was lighting a Furnace and it went out and came through the door burning EE's left forearm.
EE was turning disconnect back on for rotary valve at DC102. EE was extending left arm upward & struck metal attached to disconnect causing a laceration that required 9 sutures. No restrictions.
The plant was removing the top section of a bucket elevator and they were unaware of another crew working on the bottom section. While removing the top section that jarred a 3""x5"" compacted piece of debris within the bucket elevator. The debris landed on worker's left hand and splitting the webbing between index finger and middle finger
Employee was removing roller from belt line when roller shifted pinching arm between roller and frame of belt line.
Cement dust in the eye
Employee was lifting a mill liner that weighs approx 65 lbs when the liner slipped from employee's grip and pinched the left ring finger causing a laceration. When in the process a wet condition caused the gloves to be wet. Employee was taken to a local hospital and received stitches to close the lacerations. X-rays showed no fractures.
REPORTABLE ONLY DUE TO SUTURES - Employee was pulling strips off of a Roof bolter when employee cut employee's left index finger on the channel requiring sutures.
Thawing out a frozen water pipe. EE was pushing and pulling on a smaller pipe to break loose the frozen part of the larger pipe. No immediate injury but wrist was swollen and sore a couple days later.
An employee was cleaning the area in between the hoist motors on a dragline using a 3-step ladder. Employee finished cleaning and was descending the ladder when the left foot got stuck causing the employee to lose balance. Employee fell off of the ladder and hit their head on the elevated sub floor nearby.
While moving lubrication oil barrel, barrel slipped from one employee's hand and pinched other employee's finger.
