There was an unplanned roof on the 7-54 belt at spade 20+20.

A fall occurred in the 1st South Panel off the West Sub-main in entry #3 at SS 16+20.The fall measured 30' long x 18' wide x 6' to 8' high.

A roof fall was discovered by the fireboss in the right return of B-Panel of the old southwest mains. The length of the fall was approximately 110' by 18'5"" in width and 8' in height. No ventilation controls were affected, no employees or equipment were involved. This area has been dangered off and timbers set to prevent access.

At approximately 2:25 PM Superintendent received notice from the lead man that our bolter operator wanted us to come look at 5265-L6 because the back was starting to fall out where the last round had been taken. Upon arrival to the 5265-L6 the bolter (1621 Pluton) had been withdrawn from the heading and the back had fallen out to the intersection of 5265-L-S1.

A roof fall occurred in the NO. 4 belt entry of the 2nd North East Panel at spad 22+79. It measured approximately 50 X 18 x 8 feet. The fall fell on the second flight of belt off of number 2 unit. The unit will be moved outby the roof fall location. The fall will not be loaded out.

Employee reported a fire on the outside of cone on #3 coal mill. After the fire was noticed there was an explosion on the coal grinding circuit. At 0300 it was noticed that there was cable trays arcing above the coal mill. The power was then shut off to the circuits. Investigation continues of the root cause.

IGNITION - Occurred on 12 section, No. 1 entry approx. 10 to 12 feet from Continuous Miner. Employee went to reset power on the miner when the miner cable arced at repair splice and ignited a methane bleeder in the mine floor. The flame was extinguished with rock dust.

Around 11am 1 ladder rung was found to be broken in the #3 mancage. The ladder is used to exit the cage in case of emergency. The decision was made to evac the mine until the ladder could be repaired. The ladder was repaired by 12pm thus cancelling the evac.

First lift was being mined from crosscut between No. 5 & No. 6 entries. The pillar lift cut through into the gob, a high angle slip fell out of the lift & acres the entry above the anchorage of roof supports. The fall which was about 12' wide & over 10' high blocked 2 mobile roof supports. Roof supports were recovered by tramming them out No. 6 entry.

Fall in return discovered in #8 heading 1/2 block out by spad 2100. Operator immediately ceased mining activity when the fall was discovered. This was an unattended area of the mine.

A compressor located underground overheated and caught fire. Initial attempt to extinguish the fire was not successful and the mine was evacuated. There were no injuries as a result of this event. When the mine rescue team reached the compressor the fire was out.

Hoist #1 would not reset. MSHA was notified at 7:07 am, Evacuation started at 7:05 am and completed at 7:18 am. The hoist was repaired at 7:30 am and the miners went back underground at 7:39 am. The hoist was tested before being put back into service. To prevent recurrence the pinion brake switches was adjusted.

Hoist #1 would not reset. MSHA was notified at 3:06 pm, evacuation started at 2:55 pm and was completed at 3:05 pm. The hoist was repaired at 3:25 pm and the miners went back underground at 3:33 pm. To prevent a recurrence the pinion break switch was adjusted.

Elevator went down at 12:20 AM. The elevator was on the bottom and would not come to top. The fault screen showed motor overload. To fix the problem took a slate bar an pryed out a piece of sheet metal that was caught under the bottom of an outside ring at the base of the shaft.

An unplanned roof fall was discovered at 3-25 Mainline above the steel sets. The fall measured approximately 11' high, 16' wide and 15' long. One steel beam was affected. The beam was replaced. No ventilation or travelways were affected.

Two employees were welding and torching inside a feeder. They left to take a break and with in minutes the shop was on fire. The oxy-acetylene tanks were not shut off when they left the shop. Not conclusive what material actually ignited the fire.

#2 guide pump coupler and line. EE notified MSHA at 9:40 am, evacuation started at 9:40 am and was complete at 10:05 am. The hoist was repaired at 2:00 pm and the miners went underground at 2:05 pm. The joist was tested before being put back into service. To prevent a reoccurrence the coupler was replaced, replaced fitting and electrical motor.

Elevator went down finding that a wiring issue was located in the door switch. Wiring was repaired and elevator returned to full service.

A roof fall occurred on the West main far left roadway located at cross cut #62. The roof fall had fell in the intersection of cross cut #62 approximately 10 feet high.

During the test trip of 2 hoist the load cell fault did not trip the hoist when chairing the cage as it should have. The weight was not low enough to trip the hoist. GL Tiley was called and remotely adjusted and installed skip weight offset in load cell input data. This was essentially recalibrating the load cells allowing the 2 hoist to work correctly.

A roof fall occurred in HG5 xc 127 in entry 2. A roof fall clean up plan will be submitted and approved by MSHA before we clean up the fall.

Hoist #2 switch broke by loose scale. MSHA notified at 2:05 pm by EE. Evacuation started at 2:05 pm and was completed at 2:12 pm. The hoist was repaired at 2:20 am. The miners went back underground at 8:00 am. From now on will check loose scale in the #2 shaft.

Rockburst occurred in the 15-Stope 6400 West. Burst went with the round when it was blasted at 17:15. Damage to the ground and ground support was found by the next shift (shift foreman)when they entered the area to investigate after the seismic event was picked up by the local monitoring system. Incident originally reported as per 57.3461

At Approximately 12:55pm the substation that supplies power to the Coal Fork Portal caught on fire. The cause was unknown.

Area inby fan house had 2 mud seams come together with freezing and thawing weather. Area 3 ft. x 9 ft. x4.5 ft. thick raveled out and fell. Did not block air way or travel ways.

An accident (unplanned roof fall) occurred at this operation, between the crosscut #29 and crosscut#30 of the #11 seam. MSHA was notified within 15 minutes of the roof fall being discovered, and issued a 103(k) order upon inspection. an action plan was developed by mine management to provide adequate supplemental roof support in the area.

A on road truck caught fire overnight, causing another truck to set fire, as well as a building. No employees were on site at the time of the fire.

Main Slope Hoist lost power due to a faulty e-stop switch. Power was restored at approximatley 1:45pm.

Hoist #2 would not reset. Evacuation started at 7:20 pm. Evacuation was completed at 7:22 pm, the hoist was repaired at 7:50 pm. The miners went back underground at 8:00 pm. No injuries occurred. To prevent this from happening again the operator was informed to only stop at designated landing.

A roof fall has occurred on the 9N CM Section inby the loading point at break #32 in the #3 entry.

An unintentional roof fall occurred in F26 Longwall section just inby 15 wall in the #1 entry. The fall measured approx. 9ft high by 23ft long by 12ft wide. There was no injuries as a result of this incident.

A fire was noticed burning in a grass field that had been reclaimed north of the main haul road. The high winds spread the fire quickly. Water trucks at the mine along with fire trucks from Blue, TX VFD worked to contain and extinguish the fire. The fire was contained within 45 minutes and fully extinguished with 1.5 hours. Approximately 50 acres was scorched.

Routine Shaft work was being conducted in the 14 ft. shaft. A torch was being used to remove bolts on structural members. A hot piece of metal fell down the shaft and ignited debris in the sump.

An unplanned roof fall above the anchorage zone occurred on Section 2 (entry 1, from XC 25 to 32) where retreat mining was being conducted. The fall did not impair ventilation or impede passage

EE was traveling in Mill Building elevator when it stopped at 1:40am and would not move. EE was extracted from Mill elevator at 3:20am. Thyssen Krupp elevator technician determined fault to be caused by blown fuse.

Due to frozen grease build up on the governor wheel, the Dryridge elevator malfunctioned and stopped with an employee on board.

A roof fall was discovered by weekly examiner along the old No. 3 belt panel between spads 1034 & 1043. No equip or ee's were involved. No ventilation controls were affected. Area was mined in July 2010. 6' torque tension bolts were used. The fall measured 7' in height, 18.5' in width & 45' in length. Timbers were set at approaches & dandered off. Fall will not be cleaned up.

A small fire was located in the southwest corner of the coal stockpile. The excavator was located in the quarry and had to be brought up to stockpile to dig the fire out. Fire was extinguished at 9:48 while on the phone with MSHA 1 call. High winds and spontaneous combustion are the likely cause. No worker activity is being done in this area.

At the end of the shift the hoist operator was not able to get the brakes on #3 hoist to release. The supervisor was called and they began to troubleshoot. When it was realized that it would take more than 30 minutes to correct MSHA was notified. The issue was the pumps were building pressure but the brakes would not release.

Outage on Hoist #2 due to cold weather. MSHA was notified at 3:55 am by Mike Law. Evacuation started at 3:53 am and was complete at 4:05 am. The hoist was repaired at 10:25 am and the miners went back underground at 10:45 am. Heat was added to prevent the outage from happening again. No injuries occurred.

The production hoist was not available due to a faulty electrical contactor. The crews UG were evacuated via the Keope Hoist. A full inspection and testing was completed of the production hoist electrical system and was deemed in full operational condition. Miners were allowed back UG at 1:30pm on 1/6/2017.

During the preshift/weekly examination a roof fall was found in the number 5 entry at spad 64. Roof fall 20'w X 20'l X 6' H.

An unintentional roof fall was found on 1/3/17 at approximately 6:00pm. The fall was in the #1 Entry on the West Form Mains, approximately 100' inby survey station 5219. The fall was approximately 30'L x 20'W x 10'T. Timbers were set and the fall was dangered off from all approaches.

This was an idle shift, and there was no one underground. Our examiner was performing the elevator check and when ee pushed the button for over travel the elevator went to the bottom and brought it back to the top. But, it was stuck in over travel. There was no one underground, maintenance chief was called out to make repairs. Function test was made and working correctly.

EE was reported missing and was later discovered under a rib failure several hours later. The miner was found in an abandoned section of the mine beyond a barricade berm, along the top of a 20 foot high, waste material spoil pile. The victim was recovered on 1/26/17 at approximately 3:30am. and officially pronounce dead by the local medical examiner.

A miner was injured when employee pinched the tip of employee's left little finger while working on the Fletcher roof bolter.

EE was operating Roof Bolter. EE lost suction on side of the machine. EE was in the process of trying to regain suction. EE requested for the pin partners assistance. EE asked the pin partner to apply up-pressure when EE began to yell about finger being caught. EE received severe laceration that later required amputation of the left pinky.

Tech got finger caught between a belly pan and bolt. Pinching the left index finger. The tech in an attempt to remove the nut from the bolt pressed on the washer with their fingers to bind the bolt, while removing the nut form the bolt. Causing damage to the index finger just above the first knuckle.

Employee was assisting in the process of installing waterline when the accident occurred. EE had left hand inside the waterline in an attempt to align it when another employee pushed the waterline inadvertently smashing left pinky finger between the waterline and the rib, causing the tip of the finger to be removed.

Employee disregarded direct orders during maintenance, left group and walked around to a belt, removed the safety cover and was manually rotating the belt when power was restored to the belt. Employee's left thumb was sucked between the belt and pulley causing a severe laceration. Due to the severity, the thumb was further amputated at the hospital.

Man trip hit parked track motors in blind curve causing EE to hit head on sanders mounted just behind seat.

Employee was digging in a trench. Loose piece of dirt became dislodged and struck employee knocking ee against trench wall. Impact caused injury to torso causing bruising and possible hernia.

Employee was riding in a Shuttle Car on rough road when they felt pain in lower back. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND WAS TAKEN OFF WORK ON 4/20/17 FOR A PROCEDURE AND THERAPY.

Struck by falling object

After finishing bolting the 1st turn of the left handed crosscut, employee was loading the bolter tray with roof bolts. As employee bent over to grab the bolts, a rock fell out between the pins of the bolted crosscut. Striking employee in the right shoulder area knocking employee forward.

While walking ee stumbles in an area not designated for lunch, falling and injure right side of the shoulder, thigh, back and scrape on the neck.

Struck against a moving object

EE was operating a 745C truck,& was swerving for an 1/8 of a mile. As ee approached the top of the pit ramp, ee was seen by the oncoming operator to be slumped over in truck. As ee coasted down the ramp, the truck stopped as the front left tire came into contact with the toe of high wall. An operator put the truck in park, & woke ee. EE has low blood sugar.

Fall from ladders

Individual was standing on a ladder (3') installing spad mining sights, when dismounting not concentrating, lost balance and slipped off the ladder twisting the right ankle. This did not involve any compliance issues. Personal protective equipment was being used and did not involve job proficiency, attitude or machinery.

Employee initially injured shoulder on 1/30/17 when ee slipped & fell while dragging water hose along a beltline. Diagnosed with a shoulder strain & rotator cuff damage. Did not violate any rules or regulations & was wearing proper PPE & is properly trained. Continued to work until 8/16/17 when ee began missing work due to surgery to repair rotator cuff.

Injured was hooking up a reclaim trailer to a diesel mantrip. The trailer shifted and the trailer tongue hit injured on right shin and caused a laceration that required 11 stitches.

The employee's scoop was struck by a ram car while turning into a x-cut.

A Contract employee was roof bolting in entry 3 when a rock fell out in front of the bolter ATRS. A piece of rock broke off of that rock that measured 34""x 4""x 1 to 2"" thick, and came back and struck ee on the ankle/leg area. The contract employee was transported to the medical facility and diagnosed with a hairline fracture of the fibula. EE started missing work on 1/31/17.

Employee was walking across a concrete pad when EE slipped on a small patch of ice and fell spraining right ankle. Accident was caused by the employee's failure to watch where EE was walking. The employee didn't violate any laws, policy, or procedures and is properly trained. Employee has a positive attitude for safety.

Employee was removing brush guard from the rear window of D6 bulldozer. The guard came free from bulldozer causing the employee to become over balance and fall to the shop floor resulting in personal injury.

Repairmen doing perm on scoop was removing pump motor lids. When ee went to lift and move cover ee tore bicep muscle in right arm

Struck against a moving object

EE (truck driver) was sitting in truck while being loaded with overburden and the load hit the truck bed and shook it violently causing lower back injury.

Miner was washing out under a tail pulley at the gravel plant, was standing on some ice gave the water hose a yank and slipped on the ice and fell to the ground. Smacked right eye brow on ground. If ee wasn't wearing safety glasses it would have been worst. EE received a small scratch and a bruise on right brow. No medical attention was needed.

Employee Was Working with piece of Flagstone, when another piece of Flagstone fell off pallet and struck employee's right thumb. right thumb fracture

Employee stated was assisting persons build a stopping when air pressure pushed out the stopping. A block fell striking EE on the hand. EE exited the mine and sought medical treatment. EE was referred to a specialist and was released back to work. EE worked several shifts and returned back to the doctor and began missing work 02/02/2017.

On 2/2/17 EE reported that on 1/30/17 ee struck 3rd finger on left hand with seatbelt buckle of forklift. EE was sent to doctor & diagnosed with contusion & infection of finger. No work restrictions given or time away from work. EE continued to have pain in finger & later diagnosed with bone infection of finger requiring surgery on 10/31/17 making this a reportable event.

GRM was in field maintenance shop reaching for a jack stand in a squatting position,& when ee attempted to resume a standing position, ee felt a muscle strain in lower back on the left side. EE reported back strain to nurse, and was later brought to Dr. for an x-ray and evaluation. Prescription medications ordered and out of work for four days.

Employee states that while picking up belt framing on 1B belt, stepped and turned and felt discomfort in neck. At time of report it was file only, but after Employee went to Dr. found that needed surgery. Employee didn't miss work or restricted until 4-21-2017 for surgery on Neck.

Over-exertion in pulling or pushing objects

Mechanic was cleaning up a fuel spill, attempted to pick up an oil absorbent boom that was frozen to the ground and right knee popped and was unable to put weight on the knee. Mechanic had been having problems with this knee and was receiving professional treatment prior to this incident.

While pulling on oil refill hoses the employee slipped on some oil on the PM bay floor. After slipping employee felt a twinge in knee. Employee was taken to the doctor on 1/30/17 and released full duty. On 3/1/17 employee was not able to return to work because of knee causing this to become recordable. Employee was scheduled to start physical therapy and see an Orthopedic surgeon.

Employee was using a sledge hammer to hit a tooth on the 992 bucket to remove the tooth. A small piece of metal broke off the tooth & hit employee in the thigh.

Employee was kneeling working on a conveyor approximately 24 inches off of the ground. Employee stood up and experienced pain in knee.

Employee was assisting Mill Operator in greasing the jack-shaft for the hammer-mill. After greasing the shaft EE leaned over to pick up the grease gun and yelled out in pain. Employee stated EE ""threw back out"" bending over to pick up the grease gun. Employee has a history of back problems.

Employee was walking on flat ground when employee slipped on some ice rolling an ankle resulting in fracture to the right ankle.

Mechanic was dismounting a D10 Dozer and as employee turned to face the machine and climb down employee lost employee's footing and fell.

Employee was driving on haul road at Clintwood Elkhorn, Laurel Branch Job, when vehicle hit a dip in the road and employee felt pain in lower back.

EE told safety on May 24th of back, leg and knee pain. On June 2, EE sought medical treatment. Diagnosis unclear with possibility of two separate issues or all back related. EE stated stepped in a hole January 28, felt discomfort but continued work. In May EE's pain was below the knee & up the thigh. MRI & surgical notes show degeneration/torn meniscus. Meniscectomy on 7/11.

Employee was loading a Roof Bolter and slipped and fell while carrying a handful of roof bolts, landed on knee causing a fracture to left knee cap.

Employee was de-trashing at the H-2 belt discharge area when they strained their lower back.

The injured was starting to drill the last hole in the last row of bolts when a piece of draw rock fell on employee's left hand. The rock measured 16""-21"" in diameter and was 0-2.5"" thick. The injured was working on 4 East no. 3 heading. The injured fractured employee's left little finger and 2 pins were put in employee's left ring finger.

Our employee was approaching the pressure gun/wand (approximately 5 ft. away) during startup. Apparently the trigger gun discharged hitting employee in the face, knocking employee's safety glasses off employee's face and causing an injury to the right eye. As there were no witnesses, this is employee's account.

The employee was getting some roof plates off the roof bolter when the roof bolter operator backed up the bolter and ran on the employees left foot.

Struck by falling object

When the crane was lowered to the ground when it was done being used, it came to rest in what (at the time) was thawed snow/mud. Overnight the thawed snow/mud had become frozen. When the crane operator began to use the crane, it was observed that partial ice had formed and was not cleaned as well as thought. Falling ice/mud chunk hit and broke hard hat.

Employee bent over and picked up some roof straps and felt pain in neck and left shoulder.

Employee was working near the Raymond Mill by the feed conveyor/silo and was trying to break up frozen material.

Employee was side bolting on the left side of E29TG miner. As ee connected the drill steels, ee applied up pressure which cause the steels to bend, pinching left middle finger between the steel and the ATRS jack.

A mechanic was in the process of fueling the Komatsu Trackhoe from the lube truck. EE placed the fuel nozzle and hose on top the trackhoe handrail. EE climbed up on the trackhoe, squatted down to get the nozzle, and stood up. When the mechanic stood up, EE felt pain in EE's lower back.

Employee said when employee got off Mac 8 that employee's knee went the wrong way and that employee's knee cap moved out of place. Employee was placing employee's tools in employee's belt when employee lost employee's footing.

While working with the lower radiator hoses on a DML 45 Drill the employee dislodged dirt from immediately above where employee was working. The dirt fell onto employee's face and around employee's safety glasses resulting in materials contacting the employee's Left eye.

EE was walking in the last open crosscut from the #6 entry toward the #7 entry and ee stepped on uneven mine floor, which caused ee to twist right knee. EE continued to work without restrictions until EE was placed off work on 02/03/2017 by ee doctor. EE began missing time on 02/03/2017.

Employee was in the process of removing the bolts from the right side tram motor on a Double Boom Roof Bolter, when employee's ratchet slipped causing employee to jam employee's right arm on the mine floor. This resulted in a strain to employee's right elbow. Employee continued to work up until 3/6/2017.

Employee on 4E LW at 158 shield, the shearer was cutting through gas well, pan was pulled back to hit well from TG. The shearer was mining at #168 and the infrared program was initiated, pulling 158 shield striking EE pining leg against pan fracturing tibia and femur. No compliance issues It didn't involve job skills proficiency training, attitude PPE or protective devices

Employee was taking Lo-Trac forks off of a supply car and felt pain in shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND WAS TAKEN OFF WORK ON 02/22/17 FOR A PARTIAL TEAR OF ROTATOR CUFF.

Employee was working on the long wall when the roof behind the shields fell in the gob area causing the employee to jump back and hurting right foot when employee landed.

Tracking feed belt and stone fell behind safety glasses hitting eye.

Employee was stepping off a portable ladder EE was using to power wash equipment, when EE somehow fell and hurt wrist.

Contractor employee was cutting galvanized grating with a torch. Experienced difficult breathing on way home that day. Cause of this respiratory condition was from not wearing proper respirator protection while cutting galvanized material. Did not involve rules/regulations, mining equip/systems but did involve miner proficiency and protective devices.

Operating 349 excavator and bucket of machine hit a boulder (rock). Employee to be off work a few days (rest of this week) if not better by Monday, is to return to doctor.

On 02/03/2017, a dozer operator reported that on 01/24/2017 while ee was operating a front end loader in the pit down #5 ramp ee hurt lower back causing pain as ee hit some bumps. The operator does not know when exactly ee hurt back. The doctor gave the operator restrictions which did not allow ee to operate heavy equipment.

A fully trained miner experienced skin irritation on right arm from what miner alleges was contact with B-Bond while sealing a stopping in the 5 North Mains. The accident was reported to have happened on 01/23. However, no accident report was filed until 01/26. The incident is still under investigation and this report is being filed prior to 10 day requirements to avoid violation.

The injured employee was standing at the back of the scoop. EE flagged operator to stop. While getting jumper out of the way batteries slid off end of forks falling onto employees left foot.

EE was releasing the belt clamps from side of the belt, while the other side had been released, which resulted in left thumb being struck by the belt clamps. The operator backed over the hump and jarred back resulting in a pinched nerve in back. The employee will need physical therapy and is expected to return to work on 01/27/17.

EE positioned company f-150 truck behind a reversing 980M loader without alerting or making any eye contact with the loader operator. The counterweight on loader struck the right front fender but pickup was drivable. No injury was reported by EE the day of the incident nor the next day. When EE climbed down to the bumper, EE moved EE's lunch box from the radiator down to the bumper with EE's left hand. When EE started to climb down, EE lost grip with EE's right hand before EE could gain grip with EE's left hand. EE fell approximately five feet.

As an employee was getting out of a parked piece of equipment it started rolling backwards down a small incline hitting an employee on left side knocking employee down before the other employee could stop the piece of equipment.

Mechanic was installing a radiator in a D11T Dozer. While putting the radiator into the hard nose, the hard nose fell over hitting the employee. The employee's leg was broken.

EE was carrying 40lb sump pump motor from work bench to the pickup, when the pump cord got tangled on the work bench. The EE turned to see what the cord was tangled on when foot slipped and EE fell onto the concrete floor holding onto the pump. Pump landed on left hand causing a comminuted fracture to left hand. EE will see ortho doctor on 1/9/17 for further evaluation.

The supervisor was kneeling in front of the preheat fan VFD when the electrician tripped on the r foot causing the electrician to lose balance and fall on top of the supervisor. An employee was working at the primary crusher. A propane bottle was knocked over and fell from the upper deck striking the employee on the lower back. on 1/25/2017 the employee arrived at work complaining of pain in lower back, the employee was transported to see a doctor.

At this point the shield still did not lower but ee thought all of the pressure was off. EE opened the staple lock to the slave line and it still had pressure resulted in it striking arm.

Electrician's were attempting to engage a 4160 breaker, throw would not move, risk analysis determined they would have to look at throw. Went upstream to LOTO at power source feed to breaker located in Main Sub, located the power source, Loto'ed the power source. Went back to the ER with the breaker. Opened the box and a employee came in contact with energy inside the box.

Injured was walking at the battery charging station and tripped. Right shoulder made contact with the battery ram car bed. EE felt a pain in the right shoulder that subsided through out the shift. Person was installing roof bolts. EE had put bolt in roof and was using tension gun to tighten bolt. EE put tension on the bolt and they tightened it up and in the process ee had hand on top of roof mat which tighten left hand to roof smashing fingers. Injured employee was climbing into a chute directly below the wash screen to apply a patch. The employee slipped while entering and struck their back against a protruding bolt or pad eye on the side wall. EE tore the ligament that attaches the bicep to the elbow.

Employee ruptured distal bicep tendon while removing heater from a hanger bracket in a scissor lift. Employee needs a MRI with surgery to follow. Employee is under restricted work until surgery is scheduled.

Truck driver was preparing to turn on master switch on a caterpillar 769-c haul truck, slipped and fell. EE was asked if they were ok and EE said ankle was hurting but thought it would be ok and continued to work entire shift. EE called me around 9:00 pm at home and said they had gone to doctor and they said that they had a fractured ankle and would be out of work.

Employee was climbing into back of truck to retrieve shovel and fell backwards onto the ground landing on employee's left elbow. Employee did not have pain initially but soreness increased, continued to perform regular job. The need for rotator cuff repair was diagnosed and employee had left shoulder surgery on 4/27/17.

On 1/4/17, Employee was shoveling and said EE injured EE's shoulder. EE did not report the injury until 1/11/17. EE was taken to the doctor and was diagnosed with a left shoulder strain and released that day for light duty. EE returned for a check up on 1/25/17 and was told that it may be a torn rotator cuff. MRI to follow.

While at work pouring concrete for shaft sink, EE was holding the end of a 6"" x 12 foot hose with both hands, as concrete came to the end of the hose, the hose jerked and lifted EE causing feet to come off the ground and when ee came down it caused EE to strike right forearm on a metal hopper edge.

Employee claims that shoulder began to hurt after EE returned home from work. EE alleges that EE may have strained shoulder while using a sledge hammer to drive pins. Employee was using a guide rod to align the bolt hole on a liner prior to final placement. Employee had placed their right hand on the liner to assist with guiding the liner. When the liner was going into place between the other liner, the employee hand remained on the liner and was caught between two liners causing the right proximal phalange to break near the upper knuckle.

EE was installing a 3/8"" tubing adaptor with a 3/4"" combo wrench in a tight awkward position. As EE was torquing the adaptor into place the wrench slipped & EE's hand hit the orifice plate handle on the orifice plate on the primary air.

while helping to remove a bearing off of a screen shaft, an employee was using a hammer and struck the bearing causing a piece of metal to fly striking injured employee in the lip it was thought to be a laceration, but was later found to be imbedded in injured employee's lip

Plant employee was changing out a roll on the storage conveyor belt, the roll slipped pinching the employee's middle finger resulting in a chipped bone & laceration to the end of the middle finger. The end of the middle finger received sutures, the finger nail was re-attached with sutures. The employee was given a work as tolerated release and returned to work immediately.

EE tripped over a broken pallet while doing inventory and hurt EE's hip. Went to doctor and was told to take it easy, no restriction. 7/6/17 EE said hip still was hurting and went back to doctor and is scheduled to have physical therapy.

Over-exertion in lifting objects

Employee was lifting a 50 pound 20 foot long piece of angle iron and felt a muscle pull in rib cage. Employee went to clinic and returned to work with lifting restrictions.

Maintenance man was taking out bad mechanical laces on M10-2. EE had finishing the job and was cutting the outer edge of the belt to keep it from catching. During this process ee was using a standard razor box knife when cutting toward self the knife came loose and struck ee in the lower leg. Causing ee to have to get seven stitches in lower leg

Employee was hanging curtain in the face. EE slipped & fell from the 3rd step of an 8' ladder. It caused a very slight fracture to a bone in left forearm.

Shocks on rear engine hood were broke, lube driver lifted hood while EE picked up crib block to rest hood on. Hood slipped out of lube drivers hand striking EE on the right shoulder knocking down causing a contusion.

Operator was sitting in the seat of HT353 eating lunch, while wiping crumbs off the seat jammed left middle finger on seat cushion, felt pain heard a pop, fingertip was bent

EMPLOYEE WAS SETTING TORQUE FOR WHEEL GROUP MOUNTING BOLTS USING THE RAD GUN WITH A DOUBLE SOCKET REACTION ARM. WHEN EMPLOYEE WAS ON THE LAST BOLT THEY FLIPPED THE GUN AROUND AND WHILE SETTING TORQUE THE GUN POPPED OFF AND THE REACATION ARM AND HIT THEIR FACE.

After receiving medical paperwork on 1/24/17 it was found that employee slipped and fell due to ice on the ground causing a contusion and pain in low back.

An employee was installing pipe supports for a grizzly. The supports consisted of 8"" schedule 160 steel 52"" long. As employee was installing a support, they pinched their hand between the wall of a the crusher and pipe support causing a laceration require 12 sutures to close the wound.

Operator dismounted rubber tire dozer. Slipped on ice. Employee felt they did not need any medical care at that time. On 4-3-17, employee informed company that on 3-31-17 employee received results of x-ray and MRI. Employee will be receiving physical therapy treatment on 4-10-17, for continued numbness in left arm. To date, employee has not missed any scheduled shifts.

EMPLOYEE WAS WALKING TO THE 52 BUCKET ELEVATOR AND LOOKED UP AND AT THAT MOMENT LIME WAS FALLING FROM AN OVERHEAD STRUCTURE. LIME GOT INTO RIGHT EYE. EMPLOYEE SOUGHT MEDICAL CARE AND RECEIVED MEDICATION.

Employee was doing some live work and placed employee's hand in a hangar bearing and the finger got pinched. Employee received 10 stitches.

Associate performing demo work with a jackhammer upward, a large piece of cement broke off and came down and smashed finger, the pressure created a gash.

While installing a roof bolt, a piece of rock from the core breaker ridge dislodged striking employee on the upper lip. The employee received 3 stitches and returned to work the same day.

Contractor working at Vulcan Materials facility was working on a garage door and one of tension rods fell and struck self in the hard hat knocking it off and cutting forehead. EE also cut hand. EE was taken to the ER by ambulance where ee received staples in head and stitches in hand.

Team Member was loosening coupling bolt when handle on pipe wrench broke resulting in strain in right shoulder.

A group of miners was changing the conveyor belt on number 4 belt line. A miner was injured when EE was struck on the left forearm by a belt grab. The belt grab broke left ulna bone.

The employee was using a torque wrench and pulling very hard. EE's foot slipped as EE caught self on some all-thread, it lacerated the right small finger.

Employee was stacking 8"" concrete blocks on a pallet. The employee tripped over a brattice cloth, causing self to fall. The block ee was carrying fell on fingers-smashing them between another block.

While bolting employee stopped to scale down a small basket of loose forming in the screen just below the spring line on the hanging wall. While scaling a small rock came out of the screen & bounced off the scaling bar, making contact to employee's lower lip causing a laceration, seen by site medic and referred to the ER, employee received sutures making it reportable.

EE was taking Structure apart. When EE picked up bottom roller, the rail fell out and pinched EE's finger causing a laceration that required five stitches.

Employee was working to free up a frozen tunnel feeder in C&S. While freeing the feeder employee jammed EE's right thumb against the frame of the feeder, which caused EE's thumb area to be discolored and sore. Employee went to ER after EE's normal shift for x-ray, (no fracture). Next day 3M sent EE to the clinic for a check up and to relieve blood under nail.
