AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 1ST SOUTH PANEL OFF 1ST EAST OFF MAIN NORTH AT SPAD #11+68 #2 ENTRY #1UNIT IN THE #2 DAVIS SLAM. THE FALL MEASURED APPROX 50' X 17.5' X 10'. IT WILL NOT BE CLEANED UP. THE UNIT HAS MOVED. IT IS TIMBERED OFF AND FLAGGED OUT.
ROOF FALL, THE MINE HAS EXPERIENCED A NON-INJURYROOF AND RIB FALL THAT WAS CAUSED FROM A BOUNCE IN THE D-3 TWO ENTRY DEVELOPMENT SECTION.
A ROOF FALL OCCURRED ON THE #1 UNIT IN THE 1ST LINE OF X-CUTS IN THE RETURN ROOMS IN THE 3RD NORTH PANEL OFF THE 3RD EAST STARTING AT SPAD #11+00 INTESECTION & CONTINUTING THROUGH INTERSECTIO N 12+ 00. THE ROOF FALL WAS 100' LONG, 18' WIDE AND 8' HIGH. MSHA PERSONNEL WERE CALLED AND IT WAS INVESTIGATED.
AN UNINTENTIONAL ROOF FALL OCCURRED IN #4 ENTRY,1 BREAKOUT SPAD 7860. THE FALL MEASURED 25'L X 18'W X 5'THICK AND WAS SUPPORTED WITH 48"" RESIN TRUSS BOLTS. THE AREA IS ABOUT 2800' FROM FACE.
DRIVER SAID COAL TRUCK KICKED OUT OF GEAR AND HE LOST CONTROL OF COAL TRUCK. DRIVER REFUSED THE USE OF AMBULANCE AND SAID HE WAS NOT HURT.

A ROOF FALL OCCURRED ON THE 003-0 SECTION IN THE #3 ENTRY. THE FALL MEASURED 19' WIDE X 40' LONG X 7' THICK. VENTILATION NOR PASSAGE OF PERSONS WAS AFFECTED AND NO INJURIES WERE REPORTED. THE AREA WAS CLEANED UP AND ADDITIONAL ROOF SUPPORT WAS INSTALLED.

ROOF FALL AT INTERSECTION OF #7 ENTRY. LAST OPEN CROSS CUT ON 001 SECTION. FALL WAS ABOUT 40"" THICK ROOF WAS LAMINATED SHALE AND SAND STONE.
THE LAST ROUND HAD BEEN SHOT IN A CUT AND FILL SECTION. THERE WAS A WEDGE SHAPED SECTION OF THE BACK THAT WAS NOT FULLY SUPPORTED BY BACKFILL ONTHE LEFT RIB. WHEN MINERS CAME IN THE SECTION ABOVE THE ROUND THAT HAD BEEN SHOT, AND THE ROUND BEFORE THAT HAD BEEN SHOT, THERE WAS GROUND FAILURE.

WHILE MAKING WEEKLY FIRE BOSS RUN DISCOVERED ROOF FALL IN #6 ENTRY NEAR SPAD 31637. THE FALL DIDNOT BLOCK AIR OR ROADWAYS. WILL NOT BE CLEANED UP, TIMBERED AND DANGERED OFF.
ON 4-26-00 DURING THE NIGHT SHIFT A FALL OF GROUND EVENT OCCURRED IN THE 213 DEVELOPEMENT DRIFT. THE FALL WAS LIMITED TO THE RIGHT RIB AREA, WITHOUT INVOLVEMENT WITH THE BACK. MSHA NOTIFIED BY TELEPHONE.

DURING THE PRESHIFT EXAM TWO (2) NON-INJURY ROOF FALLS WERE OBSERVED: #1 THE #4 ENTRY 20' INBY THE LAST OPEN CROSS CUT APPROX 45' LONG X 6' HIGH X 20' WIDE.
AN INADVERTANT CONNECTION OCCURRED WITH A VERTICLE DEGAS WELL IN THE LONG CROSSCUT OFF THE #2 ENTRY AT SPAD #12423 IN THE FACE. THE HOLE WAS LOCATED 2 FEET OFF THE RIGHT RIB.
A METHANE GAS FEEDER WAS ENCOUNTERED ON THE LONGWALL FACE BETWEEN NO.26 AND NO.34 SHIELD WHILE MINING THE LONGWALL FACE. THE METHANE FEEDER CAME OUT OF THE MINE FLOOR BETWEEN THE LEGS AND GOB SIDE OF THE SHIELD.
DURING THE PRESHIFT EXAM TWO (2) NON-INJURY ROOF FALLS WERE OBSERVED. #2 THE #6 ENTRY CROSS CUT TO #5 18' WIDE X 18' LONG X 60"" HIGH.

A ROOF FALL WAS FOUND AT THE 34X-CUT. SOUTH (AT #1 ENTRY OF 3 SOUTH). THE FALL IS APPROX 20'X18'X15'H IN THE INTERSECTION OF THE OLD 3 SOUTH INTAKE ENTRY.

NO EQUIPMENT WAS INVOLVED, NO PROPERTY DAMAGE.

A ROOF FALL WAS FOUND ON 4/23/00 BY PRESHIFT EXAMINER. THE FALL WAS IN A FOUR WAY INTERSECTION AT APPROACH TO #7 SEAL MAIN EAST. THE FALL WAS APROX. 17' HIGH 18' WIDE AND 20' IN LENGTH. AREA W AS MINED IN 1982 AND WAS SUPPORTED WITH 4' FULL RESIN BOLTS. INTERSECTION WAS ALSO PROPPED. PLANS ARE TO CLEAN AND RESUPPORT INTERSECTION.
ROOF FALL, BOUNCE OCCURRED, TWO TIMBERS WERE BROKEN WITH A FEW ROOF BOLTS SHEARING WHERE ROOF CAOAL NET THE ROOF ROCK. THERE WAS APPROX 2 1/2' OF ROOF COAL IN THIS AREA.

THE FALL OCCURRED AT A 4-WAY INTERSECTION AND WILL NOT BE CLEANED UP. THE ESCAPEWAY WAS REROUTED AROUND FALL. DEVELOPE
DESKINS 'B' SHAFT SERVICE CAGE- THIS FACILITY WAS TAKEN OUT OF SERVICE FOR APPROX 50 MINUTES TO CHANGE OUT THE MOTOR BRUSHES. THERE WAS NO DAMAGE TO EQUIPMENT NOR INJURY TO PERSONNEL.

A FALL OF THE ROOF OCCURED IN THE 3RD. N.E. PANEL, AT ROOM NECK #11, 60' INBY SPAD #29863, 1200 FT OUTBY #4 UNIT. THE FALL WAS 80LX20WX5'T. NO VENTILATION, EQUIPMENT OR PERSONELL WERE AFFECT ED.
A ROOF FALL WAS DISCOVERED IN THE PRIMARY ESCAPEWAY (INTAKE) AT BREAK #33 ON 3 MAINS AT SPAD #381. THE FALL WAS APPROX 30'L, 20'W, AND APPROX 10' TO 12'THICK. NO INJURIES WERE REPORTED.
A SUPPLY TRIP CONSISTING OF 2 SUPPLY TRAILERS AND BARNEY CAR WAS BEING DROPPED DOWN TO SLOPE BOTTOM. DUE TO MUCK AND WATER ACCUMULATIONS SUPPLY PERSON CONTACTED HOIST OPERATOR TO HOLD TRIP AT 3700 MARKER. THE BRAKES ON THE SLOPE HOIST DID NOT HOLD THE TRIP AND IT PROCEEDED DOWN TO 4200.

GARDEN 'B' SHAFT SERVICE CAGE- DURING THE LOADING OF A SCOOP ONTO THIS FACILITY A HORIZONTAL CHANNEL WAS ACCIDENTALLY BENT. THE CAGE WAS REMOVED FROM SERVICE UNTIL THE HORIZONTAL CHANNEL COULD BE STRAIGHTENED. THIS FACILITY WAS DOWN APPROX 35 MINUTES. THERE WAS NO INJURY TO PERSONNEL.
LOST PWER AND WHILE CHECKING HI LINE FOUND FALL ON IN LINE AREA IN 150' INBY SPAD # 150 #3 ENTRY 1 LEFT OFF 1 SOUTH. FALL IS APPROX 20'WX2 1/2' LX5 1/2'H. FALL AREA WAS BOLTED WITH 48"" RESIN BOLTS THE AREA HAS BEEN TIMBERED AND DANGERED OFF.
THE ABOVE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL ON BOX CUT MAINS FOR MMU 012-0 INTAKE AIRWAYS APPROXIMATELY 12 CROSS-CUTS INBY THE BOX CUT PORTALS IN #8 ENTRY. THE ROOF FALL WA S 30 FEET LONG , APPROX 6 FEET THICK AND 20 FEETWIDE IN AN INTERSECTION. IT IS UNKNOWN OF THE EXACT DATE OF THE FALL, BUT IT WAS DISCOVERED 4-19-00.
AN UNPLANNED ROOF FALL WAS FOUND IN THE #4 ROOM IN ROOMS LEFT OFF 1 RIGHT. FALL WAS A RESULT OF AN OVER RIDE FROM THE PILLAR AREA, OVER TOP CRIBBED AREA INTO THE #4 ROOM. FALL STARTED APPROX IMATELY 120' IN BY SURVEY STATION #3237 AND TRAVELED INBY FOR APPROXIMATELY 200' TO THE FACE OF #4 ROOM. SUBMITTED FOR IE AND BE APPROVAL.
6 SHAFT CAGE HOIST WAS DISABLED FROM 3:40 TO 5AMPUMP FAILURE CAUSED WATER TO TRIP OUT THE CAGE WHEN THE WATER LEVEL REACHED THE SAFETY SWITCH PUMP WAS REPAIRED
AT 8:00 PM THE SECURITY GUARD DISCOVERED SMOKE COMING OUT OF THE #1 PORTAL OFFICE BUILDING. A FIRE WAS DISCOVERED IN AN OLD SHOWER/ CHANGE ROOM THAT WAS BEING USED TO STORE OLD RECORDS AND FIL ES. THE CAUSE OF THE FIRE IS STILL UNDER INVESTIGATION.

BARS OUT OF POSITION ON THE THERMAL DRYER BED CAUSED A DEAD SPOT. (NO AIR COMING THROUGH.) COAL LAYED ON THE DEAD SPOT AND CAUGHT ON FIRE. WHEN FEED TO THE DRYER GOT LIGHT A SPARK WAS PULLED U P FROM THE BED & IGNITED THE DUST.

FELL BETWEEN CRIBS IN #2 ENTRY #3 ENTRY CRUSHED OUT A STOPPING APPROX 10' HIGH, 30'LONG 19'WIDE
THE #6 ELEVATOR WAS INOPERABLE FROM 11:50 AM UNTIL 1:45 PM DUE TO A SHORT IN AN ELECTRICAL CONTROL RELAY. THIS CONVEYANCE IS PART OF THE MINE EMERGENCY ESCAPE ROUTE. AFFECTED MINERS WERE INSTR UCTED TO USE AN ALTERNATE ESCAPE ROUTE UNTIL THE ELEVATOR WAS REPAIRED.
WHILE LOADING THE FINAL SKIP ON 4-12 SHIFT OF 4-14, THE SKIP DOOR FAILED TO CLOSE. WHEN TARTING TO HOIST SKIP, THE DOOR WAS DAMAGED, REQUIRING CHAGNING OF THE SKIP.

AN UNINTENTIONAL FALL OF ROOF OCCURRED WHILE RETREAT MINING WAS BEING CONDUCTED ON THE 5TH LEFT PANEL, #3 ENTRY, #7 CROSSCUT.
THE #1 PORTAL ELEVATOR MALFUNCTIONED AT APPROX 9:10 AM. IT WAS FOUND THAT A ""K"" LIMIT SWITCH WA S OUT OF ADJUSTMENT. MILLAR ELEVATOR CO. WAS CALLED OUT AND ARRIVED AT 10:45AM. ELEVATOR WAS PUT BACK IN SERVICE AT APPROX. THE SAME TIME. THE ""K"" LIMIT SWITCFH ARM WAS ADJUSTED AND THE LU AND LD LEVELING SWITCHES WERE REPLACED.

AT 11:57 AM THE AIR COMPRESSOR SUPPLYING AIR TO THE PRODUCTION HOIST BRAKE KICKED OUT. THE BRAKE SEAL WAS LEAKING AIR, BUT WHEN OIL WAS APPLIED TO THE SEAL IT SOFTENED ALLOWING THE HOIST BRAKE S TO RELEASE. THE AIR COMPRESSOR WAS RESET AND A MAN ASSIGNED TO OIL THE SHAFT AND SEAL OF THE BRAKE ENGINE. MINE EVACUATION BEGAN AT 11:57 AM.
AT 8:17 AM SUFFICIENT AIR PRESSURE TO THE HOIST BRAKES COULDN'T BE MAINTAINED TO RELEASE THE BRAKES. THE AIR COMPRESSOR FAILED. A MINE EVACUATION WAS BEGUN IMMEDIATELY. THE AIR PRESSURE TO THE HOIST BRAKES WAS RESTORED USING AN AUXILLARY COMPRESSOR BY 9:20 AM. AFTER SKIP CHECKS, THE MINE EVACUATION WAS HALTED.

DRILL OPERATOR WAS ATTEMPTING TO REPLACE THE DECK WRENCH CYLINDER CONNECTING PIN AND KEEPER (CAB SIDE) WHICH CONNECTS THE YOLK OF THE CYLINDER TO THE DECK WRENCH FRAME. THE DECK WRENCH CYLINDE R IS HYDRAULIC CONTROLLED. EE ATTEMPTED TO OPERATE THE CYLINDER CONTROL LEVER INSIDE OF CAB DOOR WITH HIS RIGHT HAND WHILE REACHING TO ALIGN THE CYLINDER YOLK WITH THE DECK WRENCH FRAME FOR C
EE WAS PUTTING UP A BOLT. HIS FOOT SLIPPED AND HE GRABBED FOR SOMETHING TO KEEP HIM FROM FALLING. HE GOT HIS HAND ON THE DRILL POT AS IT WENT UP. THE POT PUSHED HIS HAND AGAINST THE ROOF AND C UT OFF HIS RIGHT INDEX FINGER.
EMPLOYEE WAS REMOVING SOME OLD LOCKERS FROM EMPLOYEE LOCKER ROOM AND HIT HIS RIGHT INDEX FINGER BECAME CLOSED IN ONE OF THE LOCKERS SEVERING THE TIP OF HIS FINGER.

EE ALLEGES HE WAS DRIVING IN HIS TRUCK AND HE STARTED HAVING PAIN IN HIS PELVIC AREA.

A CAPSTAN WINCH WAS USED TO POSITION AND TIE-OFF BARGES UNDER A FIXED LOADING SPOUT. THE INJURES EE COULD NOT UNTIE THE MOORING ROPE FROM THE D3E-ENERGIZED CAPSTAN WHEN HE NEEDED TO REPOSITION A BARGE BECAUSE OF HIGH ROPE TENSION. THE EE USED AN UNAUTHORIZED PROCEDURE: HE STARTED THE CAPSTAN & KICKED ROPE WITH RIGHT FOOT. IT BECAME CAUGHT. ENTIRE LEG INJURED, AMPUTATED LOWER LEG.
EMPLOYEE WAS WALKING TO A DUST COLLECTOR TO CLEAN IT. HE STUMBLED ON THE BASE OF A COAL CRUSHER AND TWISTED HIS FOOT.
EE WAS SHOVELING IN THE INBY RIB OF A CROSSCUT BETWEEN THE #3 & #4 ENTRY. THE DIESEL SCOOP CAME FROM THE #3 ENTRY ENGINE END FIRST, HE HAD 5 TO 6 FEET CLEARANCE BETWEEN THE SCOOP AND RIB. TH E SCOOP PAST BY AND TURNED LEFT GOING DOWN HILL. IT STOPPED AN DSLID DOWN HILL NEXT TO INBY RIB. EE WAS MOVING OUT OF THE WAY BUT THE SCOOP BUCKET PINCHED HIS LEFT LEG AGAINST RIB.
WAS OFF LOADING REPLACEMENT ROLLERS FOR BELT AND FELT PAIN IN GROIN. HAS CONTINUED TO WORK HIS REGULAR JOB BUT DOCTOR HAS DIAGNOSED A HERNIA. SCHEDULED FOR SURGERY ON 5/18/00.

EE WAS HELPING TO INSTALL BLADE ON CO. #120 D11 DOZER. HE WAS GUIDING THE RIGHT HOIST JACK TO THE CONNECTING BLOCK, THE JACK WAS NOT ALIGNED CAUSING CONNECTING BLOCK TO FLIP BACK CATCHING HIS HAND.
EE WAS CHANGING A DUST COLLECTOR MOTOR AT CEMENT SILOS. WHEN HE BEGAN TO WALK DOWN THE STAIRS HE TWISTED HIS LEFT ANKLE.

EE WAS LIFTING AN AIR BAG (APPROX 50 LBS) OUT OF THE CRACK IN ROCK AND FELT PAIN IN HIS LOWER BACK.

EMPLOYEE WAS OPERATING A 777D UNIT ROCK IN THE #8 PIT WHEN THE OPERATOR OF THE LOADER DROPPED A BIG ROCK IN THE BED OF THE ROCK TRUCK AND JOLTED THE EMPLOYEE. HE WAS WEARING A SEAT BELT WHICH CAUSED A PAIN IN HIS LOWER ABDOMEN.
4 WEST - BRK. 45: EMPLOYEE WAS UNLUGGING TRACK TIES WITH A HAMMER, WHEN HE MADE A SWING WITH THE HAMMER; HE MISSED THE TIE AND FELT SOMETHING PULL IN BACK WITH BURNING/TINGLING IN LEFT LEG. THIS EMPLOYEE CONTINUED TO WORK UNTIL 5/2/00. HE STARTED MISSING WORK ON 5/3/00.
EE WAS HELPING TO LOAD A HYDRAULIC HAMMER BIT INTO A PICKUP TRUCK BED. THE EE'S LEFT RING FINGER GOT PINCHED BETWEEN THE HAMMER BIT AND THE FLOOR OF THE PICKUP, INJURING HIS FINGER.
CONTRACT EE WAS TRACING AN ELECTRICAL CABLE AND TRACED IT TO A CULVERT. AS HE ATTEMPTED TO PULL THE CABLE OUT OF THE CULVERT HE STIRRED UP A NEST OF HONEYBEES THAT WERE INSIDE THE CULVERT. EE SUFFERED BEE STINGS TO THE HEAD AND EAR WHICH CAUSED HIM TO HAVE AN ALLERGIVC REACTION.
EE WERE PUTTING A PIPELINE BACK TOGETHER, LIFTING THE PIPE AT AN ANGLE. THE HOIST FOOK THAT WAS HOOKED INTO THE CHAIN WELDED ON THE PIPE CAME LOOSE AND STRUCK HIM IN THE LEFT SIDE OF THE FACE.
SUBJECT WAS ASSISTING EE ON INSTALLING ANCHOR BASES AND CHAIN HANGERS. HE WAS HOLDING HIS END OF THE BASE. EE PUSHED THE BOLT UP WITH THE BOLTER, CATCHING SUBJECT'S FINGER OVER THE BASE, BETWE EN THE BASE AND THE TOP.
EMPLOYEE WAS OPERATING MINER WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE LEG. THAT NIGHT HIS LEG GOT SORE AND HE WENT TO E.R. WHERE THEY X-RAY AND SAID HIS LEG WAS BRUISED.
EMPLOYEE WAS BREAKING COAL WITH AN END LOADER WHEN THE BUCKET OF THE LOADER WENT THROUGH THE COAL, STRIKING THE BOTTOM AND HURTING EMPLOYEE'SBACK.

HE REACHED DOWN TO PICK UP THE MINER CABLE AND WATER LINE TO THROW IT OVER TOWARD THE RIB. DURING THIS MOVEMENT HIS LEFT FOOT SLIPPED UNDER HIM WHICH CAUSED HIM TO TWIST OFF BALANCE AS HE WAS THROWING THE CABLE WITH HIS RIGHT HAND. HE
EMPLOYEE WAS IN QUARRY WORKING ON 1C CONVEYER. HE REMOVED THE MOTOR COUPLING GUARD AND TIED A ROPE AROUND IT AND SAT IT ON TOP OF THE HAND RAIL.WHEN HE WAS RELEASING HIS HAND FROM THE GUARD TO GRAB THE ROPE, THE GUARD SLIPPED AND FELL CATCHING HIS RIGHT HAND BETWEEN THE GUARD AND HAND RAIL. MIDDLE, RING, AND LITTLE FINGER WERE CUT REQUIRING 11 STITCHES, REQUIRING STITCHES.
INJURED MAN WAS CHANGING DIRECTION OF TRAVEL AFTER LOADING SHUTTLE CAR AND BUMPED HIS HEAD AGAINST MINE ROOF.
WAS HELPING TO PULL A ROCK OFF TOP OF CONTINUOUSMINER WHEN THE ROCK SPUN STRIKING HIM IN THE ARM.
DRIVER EXPLAINED THAT HE FELT HE WAS PARKED ON A LEVEL SURFACE. WHEN RAISING THE BED HE FELT THE BED SHIFTING AND TRIED TO LET IT DOWN BUT IT WAS TOO LATE AND THE CAB AND TRAILER TIPPED OVER. LOOSENED BRAKE LINE AND BRAKE FLUID SPRAYED INTO LEFT EYE.
CONTRACT EMPLOYEE WAS SHREDDING POLY HOSE WHEN A 12' PIECE OF HOSE BECAME LODGED IN THE SHREDDER. EMPLOYEE DISENGAGED THE SHREDDER AND AS HE ENGAGED IT AGAIN THE HOSE WHIPLASHED AND STRUCK HIM ACROSS THE NECK AND CHEST AREA. CONTRACTOR SUFFERED SOME SCRAPES AND CONTUSIONS TO THE NECK AND CHEST AREA.
EE WALKED IN BACK DOOR AND TRIPPED ON A 1"" WRINKLE IN THE MAT. RIGHT SHOULDER HIT A DOOR FRAME.
EE WAS PERFORMING CLEANUP INSIDE THE 57"" CONE CRUSHER AT THE CLOSE OF SHIFT. WHILE SCRAPING DOWN THE SIDES OF THE CONE, A CHUNK OF DISLODGE MATERIAL FELL AND STRUCK HIM ON THE RIGHT WRIST AREA . EE DID NOT SEEK MEDICAL ATTENTION UNTIL THE FOLLOWING DAY.
EE WAS DIGGING A ROCK OUT OF THE PIT FLOOR WITH THE OK SHOVEL. THE TEETH OF THE BUCKET SLIPPED OFF THE ROCK CAUSING HIS LEG TO HIT THE SEAT ADJUSTMENT LEVER. HE CLAIMS THE SEAT FELL SUDDENLY A ND JOLTED HIS NECK. THOUGHT HE WAS OK SAW A DR ON MAY 24TH SENT TO ANOTHER DR. WHO SAYS NO ACTIVIITY FOR TWO WEEKS.

WHILE TIGHTENING BINDERS ON A FLATBED TRAILER, THE BINDER SLIPPED, CAUSING THE EMPLOYEE TO FALL OFF OF THE TRUCK, LANDING ON HIS LEFT WRIST AND SHOULDER. EMPLOYEE WAS SENT OFF SITE FOR X-RAYS, HAD A CRACKED COLLAR BONE.
EE WAS HOLDING A FLASHLIGHT FOR THE ELECTRICIAN TO WORK ON AN ELECTRICAL BREAKER BOX WHEN THE BREAKER EXPLODED, CAUSING BURNS TO 26% OF HIS BODY. HE WAS TAKEN TO ARH VIA AMBULANCE.

INJURED WAS CHANGING FUEL FILTER ON A DOZER. HIS FOOT WAS POSITIONED ON THE TRACKS AND HIS RT. FOOT SLIPPED CAUSING HIM TO FALL, CAUSING A MUSCLE STRAIN & STRAINED LIGAMENTS TO RT. LEG. HE RET URNED TO WORK UNTIL 5-7 AND RETURNED TO DR ON 5-10 TO BEGIN THERAPY.
CRANE OPERATOR WAS DOING MECHANICAL MAINTENANCE TO THE CRANE, HE WAS SETTING THE CARBURATOR WITH THE ENGINE RUNNING. THE CRESCENT KEY HE WAS USING SLIPPED FROM HIS HAND AND HIS LEFT HAND WAS C AUGHT BY THE FAN BLADES.
THE INJURED HAD JUST RETURNED TO THE FACE WITH HIS SHUTTLE CAR WHEN A PIECE OF RIB FELL FROM ABOVE INTO THE OPERATOR'S COMPARTMENT STRIKING HIM ON THE LEFT SHOULDER AND KNEE (LEG).
EE REPORTED ON 5-22-00 THAT HE STEPPED OUT OF THE PICKUP TRUCK AND SPRAINED HIS LEFT ANKLE ON 4-25-00. ON 6-2-00 EE REPORTED THAT HIS LEG WAS HURTING. DR INFORMED HIM THAT HE HAD A RUPTURED DI SC IN HIS BACK.
EE STATED WHILE LIFTING HOOD ON 3.5 LOADER FELT PAIN IN LT ARM/SHOULDER.
WHILE BOLTING THE TOP FELL OUT IN FRONT OF THE TRS. A ROCK MEASURING 15"" X 15"" X 4"" THICK STRUCK MAN ON KNEE. HIS INJURIES CONSISTED OF BRUISING OF HIS KNEE.
EMPLOYEE WAS IN THE PROCESS OF PLACING A SEQUENCE ROLLER ONTO THE FEEDER WHEN HE INADVERTENTLY STEPPED ON A MOVING CONVEYOR BELT, WHICH THREW HIM AGAINST THE TOP; THEN, WHILE IN A PANIC STATE OF MIND, HE GRABBED A TOP ROLLER WHICH, IN TURN, CAUSED HIM TO BE THROWN TO THE BOTTOM.

APRIL 28 WAS A HOLIDAY AND NO WORK WAS SCHEDULED.
WHILE EXITING THE SLEWRING OF THE HYDRAULIC SHOVEL EMPLOYEE SLIPPED AND GRABBED ONTO SOME HYDRAULIC HOSES ABOVE. HIS ELBOWS BENT THE WRONG WAY SORE DAY. SAW DR ON 5-11, PLACED ON RESTRICTED DUTY AND PRECRIBED MEDS.

EMPLOYEE WAS TAKEN TO THE EMERGENCY ROOM, THE ROCK CUT SEVERAL TENDONS AND BROKE 2 SMALL BONES IN HIS WRIST.

WELDOR ATTEMPTED TO REMOVE A WIRE FEED WELD ""SUITCASE"" FROM THE LINE #2 WINDBOX EXHAUST FAN AFTER A MAINTENANCE SHUTDOWN. HE WAS IN A BENT OVER POSITION AND ATTEMPTED TO SLIDE THE 45-50# BOX T OWARD HIM. HE IMMEDIATELY FELT A SEVERE PAIN BACK PAIN AND HAS SINCE BEEN OFF WORK FOR LOW BACK PAIN.
END OF RIGHT FOOT UNDER LOADER BUCKET WHEN LOWERED. FRACTURED BIG TOE ON RIGHT FOOT.
EMPLOYEE WAS CLEANIG THE WINDSHIELD OF HIS END LOADER WHEN HIS FOOT SLIPPED AND HE FELL AGAINST A HEADLIGHT. HE STRUCK THE LIGHT WITH HIS CHEST CAUSING A BRUISE AND 2 FRACTURED RIBS.

ON FEB 25, EMPLOYEE WAS HELPING PUT 48"" TOP STRUCTURE ON RAILS. AS HE PULLED TO PULL THE CHIVE ROPE OVER THE STRUCTURE HE FELT PAIN IN HIS LOWERBACK. DURING CHECKUP DOCTOR FOUND A HERNIA IN RI GHT GROIN AREA. HE WAORKED HIS NEXT SCHEDULED SHIFT AND EVERY DAY. THE HERNIA CONTINUED TO GET WORSE FROM REPEATED LIFTING.
EE WAS DRILLING AND LOADING HOLES HE TURNED TO LOAD HOLE AND HE FELT A SHARP PAIN OR PULL IN HIS MUSCLE. THE PAIN WAS LOCATED IN HIS BACK. NOT SURE WHETHER IT WAS LOWER OR UPPER.
EE WAS LOADING 50 POUND BAGS ONTO TRUCK WHEN A PAIN HIT HIS LOWER BACK.

WAS WORKING ON THE POWER TO A SHUTTLE CAR (DC). HE AND HIS BUDDY HAD ENERGIZED THE CABLE TO CHECK IF THE POWER PROBLEM WAS ON THE CAR ITSELF. AS HE WALKED BY THE CAR ANCHOR THE DC CABLE BLEW U P IN HIS FACE, BURNING HIS FACE AND EYES.

LABORER WAS CLEANING ROCKS FROM CONVEYOR #6 WHILE IT WAS OPERATING. THE HOE HE WAS USING CAUGHT THE CONVEYOR BELT JERKING THE HOE TO THE RIGHT BREAKING HIS RIGHT ARM AND THE HOE HANDLE WHEN HE HIT THE STATIONARY ROLLER BRACKET. LABORERS ARE TRAINED NOT TO CLEAN OPERATING EQUIPMENT.
EE WAS DRILLING, THE DRILL STEEL HUNG UP AND WRAPPED THE AIR HOSE AND WATER HOSES AROUND THE DRILL THROWING EE TO THE GROUND. SPRAINED L ANKLE.

SOME TIME DURING THE AFTERNOON OF THE 12TH INJURED GOT A PIECE OF METAL ON HIS EYE BALL. HE HAS HAD PINK EYE & HIS EYES WERE STILL SORE ADN HE DIDN'T REALIZE HE HAD ANYTHING IN HIS EYE UNTIL AROUND NOON ON 4/13. HE HAD ON HIS SAFETY GLASSES AND A FACE SHIELD.
EE WAS HELPING INSTALL A BELT SCRAPER RUBBER ON THE E-BELTLINE. HE STOOD UP ON A 5 GAL BUCKET ADN THE BUCKET TURNED OVER CAUSING EE TO HIT HIS CHIN ON THE CORNER OF A BELT ROLLER BRACKET CUTTI NG CHIN, REQUIRING 7 STITCHES.
EE SAYS THAT WHILE HE & 2 OTHER EE'S WERE ATTEMPTING TO REMOVE A TOOL BOX MODE OF 3/8"" PLATE, HEWAS HOLDING IT & HURT HIS BACK. HE FRIST BROUGHTBACK INJURY TO OUR ATTENTION 1ST WK OF JUNE. HE WASNT' SURE IF IT WAS WORK RELATED OR JUST AN O LD BULL RIDING INJURY. HE DECIDED TO GO ON HIS OWN BECAUSE HE WASN'T SURE. HE RETD 6/16 WITH NO RESTRICTIIONS. THE BEGINNNING OF SEPT HE HAD TO
NOT POSITIVE WHAT ACTUALLY CAUSED THE NECK MUSCLE INJURY. ONE THIS PARTICULAR DAY HE SET A WATER PUMP AND WAS PULLING ON A PIPE WRENCH, WHICH IS NOT UNCOMMON.

WHILE OPERATING THE EXCAVATOR, HE STOPPED THE MACHINE TO GET OUT AND CHECK HIS WORK. WHEN HE STEPPED DOWN TO THE GROUND, HE TWISTED HIS KNEE.
8E SECTION STANDING IN AN INTERSECTION WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF STRAPS AND STRUCK TOP OF LEFT FOOT AND HEAD. HE RECEIVED HAIRLINE FRACTURE TO FOOT AND 2 LACERATIONS BETWEEN TOWS. *LOST WORK DAYS BEGAN 4/13/00.
EE WAS LIFTING A 33 LB CHOCK BLOCK WHICH WAS UNDER THE FRONT WHEEL OF HIS 40 TON CAT TRUCK. HE TURNED TO PUT THE BLOCK ON THE HOLDER LOCATED ON THE FRONT OF THE TRUCK AND HE FELT A SHARP PAIN IN HIS LOWER BACK. HE WORKED THE REST OF THE DAY. ON 4/13 HE WORKED UNTIL APPROX 10 AM AND THEN WAS SEEN AT A LOCAL CLINIC.
EE WAS CROSSING OVER THE CONVEYOR BELT ON THE SECTION'S REM (METAL) FROM THE RETURN SIDE OF THE BELTLINE. THE BELT WAS OFF AND THEN SOMEONE TURNED THE BELT ON CAUSING THE EE TO FALL DOWN ONTO HIS RIGHT SHOULDER. EE HAS BEEN COUNSELED ON THE BELTS. IT IS AGAINST COMPANY POLICY TO CROSS BELT.
EE GOT OFF MANTRIP OUTSIDE, TRIPPED OVER HIS FEET AND FELL.
WHILE HE WAS OPERATING AN EXCAVATOR, IT TURNED OVER, CAUSING A NECK STRAIN.

EE WAS WALKING DOWN THE STAIRS BY THE 1ST NORTH EVAPORATOR AND TWISTED HIS KNEE. HE HAD SURGERY ON 4/14/00. OUR FIRST REPORT OF THE ACCIDENT LISTED IT AS NON OCCUPATIONAL. IT WAS DETERMINED ON THE WEEK OF 5/1/00 THAT IT WAS AN OCCUPATIONAL INJURY.
