THE AREA HAD BEEN SUPPORTED BY 8 F T TORQUE TENSION BOLTS SUPPLEMENTED WITH WIRE MESH. THERE IS EVIDENCE OF SLICKEN-SIDED MATERIALS IN THE UPPER PORTIONS OF THE FALL.

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE TAIL GATE OFF OF THE 8TH WEST LONGWALL, #3 ENTRY OF THE 7W AT 4460'WEST. THE FALL WAS APPROX. 25' LONG, APPROX. 18'WIDE AND APPROX.6-8'THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF MINERS. THE FALL WILL BE MINED PAST.
THE 600 AMP AC BREAKER FOR THE GARDEN ""B"" SHAFT AUTOMATIC ELEVATOR HAD A FAULT, WHICH RENDERED THE ELEVATOR INOPERATIVE IN EXCESS OF 30 MINUTES.
DURING A SEVERE STORM A DIRECT LIGHTENING STRIKE HIT THE COMPANY'S SUBSTATION CAUSING A FIRE THAT BURNED UNTIL APPROXIMATELY 3:30 P.M. WHEN THE FIRE DEPARTMENT GOT IT OUT. FIRE CONFINED TO THE OCB UNIT. NO LIGHTENING ARRESTOR DAMAGE ON EITHER SIDE OF SUBSTATION. THERE WERE NO INJURIES FROM THE FIRE.
SLOPE CAR WAS REMOVED FROM SERVICE AS A RESULT OF A DISTORTED PLACE IN HOIST CABLE. ROPE WAS REPLACED ON 1-4-2003 AND SLOPE CAR WAS PUT BACK IN SERVICE.
A ROOF FALL OCCURRED IN THE N0 1 ENTRY OF THE 19 EAST (#3 UNIT). IT FELL IN A PILLAR LINE LOCATED BETWEEN SPADS 7737 & 7852. THE FALL MEASURED APPROX. 100' LONG X 18' WIDE X 1042' HIGH. THE UN IT WAS IDLE AT THE TIME.
HAULER OPERATOR WAS MAKING HIS FIRST LOAD OF THESHIFT TO THE LOADING STATION FROM DIII PIT ON #32026 HAULER. HE WAS APPROX.2 MILES FRO THE PITON #7 RAMP HAUL ROAD WHEN HE NOTICED FLAMES AROU ND THE HITCH AREA OF THE HAULER. HE STOPPED THE MACHINE SET HIS PARKING BRAKE AND ATTEMPTED UNSUCCESFULLY TO EXTINGUISH THE FIRE WITH A 20LB EXTINGUISHER. WATER TRUCKS CALLED FIRE PUT OUT 8:45

THE SLACK ROPE SAFETY SWITCH FAILED ON THE MAN/ORE HOIST CAUSING THE HOIST TO STOP. NO ONE WAS IN THE CAGE AT THE TIME. THE SLACK ROPE SWITCH WAS REPAIRED AND THE HOIST RETURNED TO SERVICE.

A FALL OF ROOF OCCURED IN THE 2ND WEST PARRALLELS IN THE #5 ENTRY AT X-CUT #6 THE SIZE OF THE FALL WAS 20'LONG X 20'WIDE X 8'THICK. CAUSE WAS UNDETECTED BREAK IN THE ROOF STRATA.
STARTED A #2 LEFT BREAK CUT ABOUT 15' DEEP. TOP STARTED TO WORK BEFORE ROOF BOLTER GOT TO IT. WE LET IT WORK AWHILE. IT FELL ABOUT 4' THICK AND PULLED ROCK FROM AROUND A COUPLE ROLLS OF BOLTS IN THE INTERSECTION. AREA HAS BIG SLIPS SO WE TIMBERED AND DANGERED IT OFF AND CUT AROUND THE AREA.
HAD SMALL FALL OF GOB AND HORSEBACK AT 19+00 ON #1 UNIT- NEUTRAL ENTRY. 20'X 3'X 8'.
A ROOFFALL IN THE 5TH WEST PANEL #3 ENTRY BETWEEN X-CUT #31&32 APPROX. 40'X19'X6'. 100' OUTBY THE FACE. THE AREA HAD BEEN SUPPORTED BY 6'TENSION REBAR BOLTS AND WAS CAUSED BY AN UNINTENTIONA L SLIP.

AFTER TROUBLE SHOOTING FOR 20 MINUTES OR SO, WE CALLED E LEC ENGINEER. HE TOLD US THINGS TO BE CHECKING WHILE HE WAS ON HIS WAY. WE FOUND MOTOR OVERCURRENT RELAY HAD TRIPPED. WE RESET, RAN CAGE BACK TO BOTTOM, LET PERSON OFF & RAN TEST ROUND & BEGA

ON 12-17-02 THE MINE EXAMINER WAS CONCLUDING HIS WEEKLY VENTILATION CHECKS. HE ENCOUNTERED THE ROOF FALL. IN THE #3 BLEEDER AREA. #3 MAINS. IN #2 ENTRY 1 +CUT INBY SPAD #2687. THE FALL WAS APP ROX 70' LONG X 18' WIDE X 4' THICK.
WHILE PILLARING, A PARTIAL PILLAR FALL OCCURRED WHILE MINING THE SECOND LIFT OUT OF THE NUMBER 2 BLOCK, IT FELL BESIDE THE MINER WEDGING IT TO THE RIB. THE EQUIPMENT INVOLVED WAS A JOY 14-15 M INER. NO VENTILATION WAS DISTURBED. NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.

THE ROLL-BACK SWITCH ON THE BRAKE CAR MALFUNCTIONED. THE ROLL-BACK SWITCH WAS REPLACED AND THE BRAKE CAR WAS PUT BACK INTO SERVICE AT 3:30 PM.

FALL WAS IN A FOUR WAY INTERSECTION THAT WAS MINED 02/27/97 & HAD BEEN SUPPORTED WITH 6'FULL RES IN BOLTS. NO INJURY RESULTED, BUT 40' OF POWER CABLE AND WATER LINE WERE LOST. THE FALL WILL NOT BE CLEANED UP.

AN UNPLANNED ROOF FALL AT THE PEGASUS MINE, ON #5 BELT. FALL CAME FROM BELT ENTRY OVER TO TRACK ENTRY AND STOPPED WHERE CRIBBS HAD ALREADY BEEN INSTALLED. AROUND 2' THICK AT TRACK ENTRY UP TO 8' ON BELT ENTRY, CALLED MSHA MARTIN OFFICE, ANDREPORTED IT TO EE, THE MORNING OF FALL, OTHER WAS SENT DOWN TO CHECK ON FALL.
A ROOF FALL OCCURRED IN THE 9TH EAST, #4 ENTRY, AT #160 CROSSCUT. THE FALL WAS 20' LONG, 30' WIDE, AND 9' HIGH. IT DIDN'T AFFECT VENTILATION, TRAVEL, DAMAGE EQUIPMENT, OR CAUSE ANY INJURY. THE AREA WAS BOLTED WITH 8' POINT ANCHOR BOLTS.

A FAULT INTERSECTION IN THE 2100 L CAVED ABOVE BOLT ANCHORAGE IN THE BACK. THE STRUCTURE WAS WEDGE-SHAPED AND CONSISTED OF ABOUT 50-75 TONS OF ROCK. 7 FT LONG SPLIT-SETS WERE USED, IN CONJUNCT ION WITH CHAIN LINK WIRE. ONE OR MORE SPLIT-SETS HAD RUSTED BADLY. THIS DRIFT WAS DEVELOPED IN EARLY 1995.

AFTER THREE WEEKS OF BEING TREATED FOR GROIN MUSCLE STRAIN, IT WAS DIAGNOSED AS A RIGHT INGUINAL HERNIA. SURGERY IS SCHEDULED FOR TUESDAY, FEBRUARY 4, 2003.

EMPLOYEE WAS CARRYING SOME HEAVY PARTS ON AN UNEVEN SURFACE IN #2 COOLER AND SUSTAINED A STRAIN TO LOWER LEFT LEG AND KNEE. NOTIFIED SUPERVISOR ON 1/9/03.

EE CUT A TREE ABOUT 6 INCHES IN DIAMETER. THE TREE FELL ACROSS A TREE LYING ON THE GROUND. THE BUTT OF THE TREE HE CUT SPRUNG UP AND STRUCK HIM IN THE JAW.

EE WAS TRYING TO GET A PIN OUT OF A LONGWALL PAN THAT WAS LEFT ON A SHIELD HAULER. HE SAID THE PIN WAS HARD TO GET TO & WHILE USING A HAMMER TO REMOVE IT, HE STRUCK HIS RIGHT THUMB. HE HAD A CONTUSION & A FINGER SPLINT WAS APPLIED. THE TIP OF HIS THUMB BONE WAS FOUND TO BE FRACTURED BY THE DOCTOR. HE MISSED WORK SATURDAY 12/14/02 BUT RETURNED TO WORK MONDAY, 12/16/02.

IN A LITTLE WHILE HIS KNEE SWELLED. HE WENT TO DR AND HE TOLD HIM THAT THE BUMP EVEN THOUGH SLIGHTLY BUMPED, IT ACTIVATED THE INFECTION. HE HAD TO HAVE ANTIBIOTICS.
EE WAS MOVING A STONE WHEN HE LOST FOOTING ON SNOW & ICE. HE FELL ON GROUND AND TWISTED HIS BACK.
EE WAS PICKING STONES UP OFF THE GROUND AND PUTTING ON PALLETS WHEN HE FELT A BURNING SENSATION IN THE BACK OF HIS NECK AND LEFT SHOULDER AREA.

THE INJURED PERSON WAS CROSSING BELTLINE WHEN HE FELL AND HURT HIS BACK AND LEG.

B2#2 BELT LINE - EE WAS REMOVING A CONVEYOR SHAFT OUT OF THE L.S. CONVEYOR MOTOR ON #69 MINER. WHEN HE PULLED THE SHAFT OUT, IT WAS OIL SLICK. THE OIL FELL ON HIS RIGHT UPPER/MIDDLE ARM CAUSIN G 1ST & 2ND DEGREE BURNS.
WORKING TO DE HOSE A SHIELD DURING LONGWALL MOVE. HE WAS PRYING ON A STAPLE LOCK & WHEN THE STAPLE LOCK CAME SUDDENLY FREE HE SAID HE FELT A PAIN IN HIS RIGHT SHOULDER. HE CONTINUED TO WORK UN TIL DEC. 27TH, 2002, WHEN HE WAS DISABLED.
EE STATED THAT WHILE HELPING MECHANIC REPLACE A PANEL ON DOZER HE STEPPED BACK AND FELL FROM THE DOZER CAB LEVEL PLATFORM TO THE TRACK OF THE DOZER.
EE WAS USING A RACHET TO REMOVE A BOLT FROM THE LEFT DRUM. THE ALLEN PLUG BROKE CAUSING HIM TO SLIP AND FALL AGAINST THE KNEE OF ANOTHER EE WHO WAS WALKING BY AT THE TIME.
THE EMPLOYEE HAD SET A HEAD DRIVE & CLIMBED UP ON TOP TO SET A CHAIN & FELL OFF OF HEAD DRIVE & HIT CRIB BLOCK & TWISTED LEFT ANKLE.
HE AND ANOTHER PERSON WERE LIFTING A MINER CABLE WHEN HE BENT OVER TO PICK UP THE MINER CABLE TO THROW IT INTO THE HANGER, HE FELT A PAIN IN HIS LOWER LEFT SIDE.
EE WAS PULLING ON A WRENCH WHEN HE FELT SOMETHING POP IN THE LITTLE FINGER OF HIS RIGHT HAND. HE FINISHED HIS SHIFT AND WENT TO THE EMERGENCY ROOM THE NEXT DAY WHERE HE WAS DIAGNOSED WITH A BR OKEN FINGER.
EMPLOYEE STATED HE WAS MOVING A BARREL OF COMPRESSOR OIL TO SERVICE THE DRILL. WHILE MOVING THE BARREL HE TWISTED HIS LOWER BACK. HE EXPECTS TO BE OFF WORK FOR AT LEAST ONE WEEK.

EMPLOYEE WAS INSPECTING A SNUB ROLLER ON NO. 7 & NO. 5 TRANSFER POINT. HE WAS USING WATER TO SPRAY ROLLER OFF, SPRAYED WATER ON ONE SIDE, WENT TO THE OTHER SIDE & A PIECE OF ROCK FELL FROM RIB STRIKING HIM ON THE SHOULDER.

EE WAS ASSISTING IN REMOVING AN IDLER FROM THE CONVEYING SYSTEM. HE PLACED HIS HAND IN A PINCH POINT. THE IDLER SMASHED HIS FINGER RESULTING IN A FRACTURE TO THE TIP OF THE FINGER.
EE WAS BOLTING TOP RIB CAME LOOSE ABOUT 8 FT LONG, 4 FT HIGH, SLIPPED OFF KICKING A PIECE OF COAL AND ROCK LOOSE 57 IN LONG, 12 INCH THICK, PUSHING DRILL OPERATORS RIGHT LEG UNDER DRILL HEAD.

AS HE STARTED TO MOVE TOWARD THE MIDDLE OF THE MINER IT'S BACK END MOVED OVER & PINNED HIM BETWEEN THE RIB & THE TRAY ON THE MINER. THIS INCIDENT TORE SOME LIGAMENTS IN HIS CHEST AREA & BRUISED HIS BACK.
WHILE DESCENDING THE HAUL TRUCK LADDER AT THE END OF SHIFT, EMPLOYEE LOST HIS GRIP ON THE LADDER HAND RAIL AND FELL APPROXIMATELY 5 FEET TO THE GROUND, LANDING ON HIS BUTTOCKS. HE SUFFERED A F RACTURED WRIST AND A CRACKED VERTEBRAE IN HIS BACK.

EMPLOYEE WAS WATCHING THE AUTOMATIC WELDER BUILD-UP ROLLS ON CRUSHER. WHILE LOOKING AT THE WORK HE RECEIVED RADIATION BURNS TO BOTH EYES. HE DID NOT SEEK MEDICAL TREATMENT BUT COULD NOT COME T O WORK THE FOLLOWING DAY.
EE WAS USING A 5' METAL PRY BAR TO REMOVE A PALLET CAR AXLE. THE BAR SLIPPED CAUSING THE EE TO HYPEREXTEND HIS LEFT SHOULDER. HE WAS DIANGNOSED WITH A ROTATOR CUFF TEAR ON 1/28/03. SURGERY WAS CONDUCTED 4/23/03

EE WAS INSTALLING TRUSS BOLTS IN THE TRACK ENTRY WHEN DRAW ROCK FELL FROM THE TOP. THE ROCK STRUCK HIM IN THE BACK OF HIS NECK.
