A roof fall occurred along 1R #2 beltline XC #9 Entry #2. Entry had a clay vein running along right side rib line through the intersection. A second clay vein entered the intersection from the left rib joining the first forming a cantilever in the intersection. Rubble from the fall compromised a Kennedy stopping. There was no injury or property damage with this occurrence.

Roof fall on Cumberland West Belt 30xc on the belt line. Fall was above the anchorage.
An employee became trapped in the Finish Mill elevator at approx. 03:00 am while going to the 3rd floor. The elevator stalled between the 1st and 2nd floor. Otis elevator was contacted and assisted in removing the uninjured employee. The third floor door contact was the issue per Otis.
At approx. 730 a contractor became trapped in a elevator. Otis was called and assisted removing the uninjured contractor from the elevator. the Otis tech advised us the first floor door contact was not making good contact with the door sensor causing the elevator to shutdown.
A roof fall was found on 10/27/2021 during the weekly examination this fall is located in the Right Fork No.1 panel. This fall has been dangered off from all approaches.
ROOF FALL - A roof fall was found by examiner conducting 7 day exam at x-cut 142-1/2 in the North Main #6 entry air course. The fall was approx. 30 feet long, rib to rib and 10 foot high. No methane at location and proper air flow.
At approximately 12:30pm on 10.25.21 a washout was reported at the TR underground crusher pad (located on the surface north of the TR Underground admin building). It is believed that this washout of the crusher pad was caused by the excessive rains received in the region. Operations is developing a repair plan that be conducted once the area has dried out.
Wheel motors of HTK240 caught fire. Operators cleared truck; sustained no injuries. WTK applied approx 2000 gallons of water. Fire was not able to be extinguished completely. MSHA hotline was called at 30 minutes. EE called back with instructions to seize extinguishing efforts, evacuate area, issue J Order 9613334. Quantification of damage-No injuries/Property damage HTK240
During the daily exam of the hoist an issue was found with a pressure transducer when the static brake test was completed. The hoist was taken out of service until the pressure transducer could be repaired. The hoist was repaired and tested before being placed back into service.
The pump failed causing one of the fuses to blow in the pump starter, and because the starter did not trip it caused the motor to single-phase which caused the #3 Station Sub 600 amp disconnect to blow a fuse. The 600 amp disconnect in the Sub on the #3 station is burned up and needs to be replaced and 700HP pump burned up where fire started.
On Friday, October 22 a 992G caught fire, operator was able to escape safely. The fire suppression was not able to be set off manually, so the fire department was called.
A roof fall was found in the examiner's weekly return examination route. The fall was located in Entry #1 between #108 and #109 crosscuts of the 2nd Panel North off the 1st Main East. The fall was approximately 8 feet high, 20 feet wide, and 50 feet long.
A face ignition was reported on the H2 section while mining the #3 Return entry Survey Spad #66+78. During the investigation Sulphur balls were found in the face area. MSHA cited two missing/worn bits on the left side of the continuous miner head. No injuries occurred.
An unplanned roof fall occurred in the 4 west area at 26 x-cut. Adverse geological conditions caused the roof to separate and break. The roof fell 10ft from the track entry (#3 entry) through the crosscut to the belt, bridged over the belt entry and continued falling in the crosscut from the belt (#2 entry) towards the adjacent entry (#1 entry).
NGM Loader 3935 was tramming a Z50 truck battery from the mine to the 17 shop. While on the MAD decline, the battery caught fire and visible flames were observed. The loader operator was able to set the battery down in the drift and back away. It is unknown, at this time, whether or not the battery pack is still on fire.
A rockfall occurred on the 2nd Southwest belt at spad 32+50 in the #5 Entry. The fall was 45x18x9. There were no injuries from the rockfall.
Two contractors stuck in pre-heat elevator (entrapment)
A non-injury roof fall was discovered on the No. 5 Belt Conveyor in the No. 19 cross cut, spad 1337. Fall measured approximately 50-60 feet in length, 20 feet in width and approximately 6 feet high.
The plant tower elevator stopped working due to a bug that was blocking a position sensor which caused it to stop. An employee was in the elevator and notified the control room per procedure informing them that they were stuck just above the landing. The employee was in the elevator for more than 30 minutes and the event was called in. No injury associated with this event.
A roof fall was discovered at 15-13 between the track and the belt.
While performing a stack test, Business Partner employees raised the Genie man-lift to approximately 60' and began to have control problem. Inside the bucket, they tried to get the controls to work, unable to do so, the supervisor tried to get them down from the ground controls. They were unable to get the bucket lowered, NGM employees used a crane bucket to get them down safely.
At ~ 6:35 an employee reporting to work saying there was coal coming out of the stack. Around 10:00 PM CO sensors were found offline, at about 10:45 a smoldering fire was discovered, and Buffalo VFD was called.
A roof fall was discovered on 10 track at 52 wall.
Pre-check of manlift performed before operation; functioned normally. After raising basket to operating height, basket controls and ground controls stopped functioning. Mechanic tried to repair but that didn't work so employees were rescued from manlift using a crane. No injuries, illnesses or damage occurred.
An unintentional roof fall occurred in 5B LW #3 Entry 25-26 Wall. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof. The fall area measured approximately 65' L x 16' W x 10' H.
The hoist ropes came out of the boom point sheeves on the BE1570 dragline when the bucket contacted the spoil pile. The operator tried to lower the bucket which created heat/friction between the ropes and the structure, igniting nearby grease. A fire started that was not extinguished within 30 minutes of discovery.
SC Panel 3 - Fall in return located in No.5 and No.6 starting 1 break inby regulator line extending into Gob. Also located 1 break inby 1306 spad, extending through right hand break to intersection of number 8 entry.
SC Panel 2 - Fall in return located 1 break outby spad number 1382 to outby spad number 1401. The area of where this fall is located has been sealed.
The mine examiner was working on the tracking system in the intake entry at the #7 cross cut. This area is not a normal travel way. The roof fall was 4' H, 20' wide and 40' long approx. No interruption was shown on the fan chart.
An unintentional roof fall occurred in 3 East Mains #2 Entry 80 Wall intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof. The fall area measured approximately 40' L x 50' W x 16' H.
The SP Elevator was having issues with the door switches and was down longer than the required time. The repairs were made to the switches and the elevator was tested prior to being put back into service. MSHA was notified via the hotline and an email was sent to the FOS.
Some timber failed and rubble rolled out into roadway impeding the travel way.
Unplanned roof falls occurred in the cross cuts between entries #6 and #7 and entries #7 and #8 at cross cut 10 on the 1H3 Panel. The falls were found at approximately 2:15 am on October 2, 2021. The roof falls are estimated to be 6 feet high. A shuttle car was located beneath the fall that occurred in the cross cut between entries #7 and #8.
The roof fall occurred in the No. 5 entry between X-cut No. 26 and x-cut No. 27 on the DS-2 mains. The roof fall is confined to the No. 5 entry. The fall is approximately 12' long, 10' wide, and 5' thick/high. Ventilation was not effected and the fall was cleaned up and re-supported.
Had Roof Fall at Cross Cut #46 in the track entry that measured 8Ft thick, 80 to 90 FEET long and 18 Ft wide.
The roof fall happened in the No.4 intake entry at x-cut No. 28 on the DS-2 mains. The roof fall is approximately 30' Long, 19' Wide, and 8' thick/high. Ventilation was not affected. The primary escapeway has been re-routed to the No. 5 entry starting at x-cut No. 26 on the DS-2 mains.
An employee was fatally injured during maintenance work on the hopper. EE was operating a CAT forklift in close proximity to a Volvo excavator. EE exited the vehicle without notifying the other employees working in the area. Other employees stated they heard a ""pop"" and found the forklift operator unconscious next to the fork of the forklift.
Employee was working on removing a pin from a leveling cylinder on the back of the rock truck. While the employee was performing this work, the haul truck bed fell on top of the employee.
A miner entered a surge bin to free stuck material. The material collapsed and engulfed the miner.
Employee climbed on belt to clear a clog, EE was not tied off, and EE did not LO/TO. The plant operator started the plant while employee was still on belt. Employee grabbed the drive pully to catch themselves and the belt cut first and second finger. Second finger was repaired in surgery. The first finger was removed from fingernail down during surgery.
Miner was attempting to reposition a continuous miner head being hauled on the track using multiple jacks when one of the jacks slipped pinching the tip of EE's left ring finger between another jack. Resulted in amputation down to first knuckle. (60 days).
Employee was walking from the tailgate towards the head-gate when employee was stepping from shield 105 to shield 104 a rock fell from in-between the shields and landed on employee's foot. This caused a laceration between the two small toes on right foot and fractured the small toe.
Employee was using a come-along to lift an ATRS jack over the rib door on the miner when EE pinched EE's left hand between the jack and door. EE received 15 stitches.
Employee was pulling slack for a CM cable and felt pain in a knee Employee did not miss work until 11/17/2021 when they were diagnosed with a tear in the knee that will require surgery.
After completing some welds wearing safety glasses and shield, employee moved a ladder. Dust fell on EE and EE got something in their eye. EE used the eyewash station and returned to work. EE's eye got irritated later in the shift. EE washed eyes again and the manager told EE to see a doctor. The employee left work and received treatment.
The injured EE fell in the shower after tripping on their sandal. The EE lost balance, fell forward, and struck their face on the shower nozzle. The EE received stitches above eye and a fracture to their face. EE began missing work 11/1/21.
Operator was utilizing a bob cat walk behind for clean up. Operator was dumping product of mud in the sump when the walk behind became unstable bucking the operator off the back of the machine landing on their back.
Note: The employee did not begin missing work until 11/2/2021. Description: After drilling the inside bolt hole the employee inserted the glue and as EE was inserting the bolt into the hole EE felt a pop in EE's left shoulder.
A Miner had contact with a low voltage control wire and received a shock. EE was taken to a local medical facility for evaluation and released. EE will be off work for 1 week pending a follow up appointment.
A miner driving a UTV 4WD personnel carrier struck a contractor that was walking in the crosswalk between the Patras building and office buildings. The contractor felt pain on EE's left elbow, as well as the left butt cheek, also stated feeling pain on the back of EE's head

The contractor felt pain from left elbow to fingers specifically left ring and pinky fingers, they also felt pain on left thigh as well as the back of the head.
SCOOP OPERATOR HAD BEEN SCOOPING RIBS, HAD STARTED TO CLEAN THE CORNER AND WAS PARKED. IE STEPPED INTO THE SCOOP BUCKET TO TELL THE OPERATOR EE WAS NEEDED ON THE OTHER SIDE OF THE SECTION. A PORTION OF CAP ROCK BECAME DISLODGED, FELL INTO THE BUCKET AND STRUCK IE's LOWER LEG.
A fully trained miner was dragging a ventilation tube when they stepped in a hole and twisted their leg. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was getting bag out of Pickup truck. Employee stepped on tire to reach bag. Upon lowering back to the ground, the employees leg was injured.
Crew was lowering material down to the employee with a bucket and a rope. The handle of the bucket broke and fell, bouncing off of the wall and striking the employee. When the bucket struck EE, it knocked EE off of a platform causing EE to fall on a pole.
Employee was guiding/turning the rock deflector into position as the ATRS jack was being pressurized. They were mining through a clay vein, which had caused bad top conditions. The pad on the ATRS went into a cavity in the top, allowing the rock deflector to pinch EE's hand against the top.
Employee felt dizzy while getting out of 916 water truck. Grabbed steering wheel as lost balance and felt a pain in the right arm/shoulder.
While operating the shear EE stepped in a hole between relay bars and twisted EE's left knee.
Employee was foaming the top of an overcast, and was walking backwards and fell through a hole in the overcast landing on right ribs and elbow. Employee started missing 11/2/21 waiting on an MRI.
Employee was walking down Long Wall face and stepped in a hole twisting their knee.
While assisting pulling belt, a miner was struck above the right eye by a shive rope when it broke. Injury was not the result of a training or compliance issue.
Employee was directing a scoop when EE slipped and fell, striking EE's left forearm on a crib. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
The 2 people involved were removing a 4"" piece of pipe. The pipe was inspected & thought to be secured to 2 braces by welds. There were no welds securing the pipe to the braces. The injured person was on a ladder using a saw, when the pipe was cut, the pipe struck the leg of the ladder resulting in the fall.
Employee was doing plant repairs on 10/26/21. IE and co-worker were working together on different repairs all day. Co-worker was doing the welding, IE was helping cutting steel, grinding, and general labor. Later that evening IE had eye irritation and went to the ER. Doctor took IE off work 10/27 and 10/28.
Greasing upper bucket pin on front end loader. Worker was standing on a step approximately 24"" above ground level. Worker's foot slipped and worker landed with back on the bucket.
EE WAS ROOF BOLTING WHEN A PIECE OF DRAW ROCK FELL AND STRUCK RIGHT FOOT/ANKLE. EE SAW A DOCTOR AND WAS ALLOWED TO CONTINUE WORKING UNTIL 11-18-21 WHEN IT WAS DETERMINED THERE WAS A FRACTURE OF THE END OF THE FIBULA. EE BEGAN TO LOSE TIME ON THAT DATE AND IS CURRENTLY OFF WORK.
Employee was cleaning on the belt line and had left hand on the bedrail and was struck by a rock that had fallen from the roof, causing a contusion. Compliance with rules/regs PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
It has been muddy on the mine. The operator was shoveling the tracks of the dozer and was pushing hard. EE then felt something in the right knee pop.
Mechanism of injury occurred on 10/26/2021. EE knelt to pick up debris on belt line walkway. Felt discomfort in right knee. Seen at clinic treated first aid. 6/17/22 Pain present. EE diagnosed by MRI slight tear to ACL. No restrictions. 7/7/21 arthroscopy to right knee.
Employee was walking in the area of the traveling water screens and the grating fell through the support causing employee's right foot to go into the resulting hole. Employee states that employee's left leg kept employee from falling all the way into the hole. Right ankle discomfort and placed out of work.
The operator was dumping bauxite material at the C4 area. After unloading the material at the C4 area, the employee admitted that employee forgot to lower the bed of the haul truck and struck a conveyor belt.
A fully trained miner was changing a bottom roller when it fell and struck them in the foot. The injury was not the result of any compliance issues nor were any protective guards involved.

4900-310 switch - move supplies by timber truck. Derailed timber truck; had to use draw bar to lift each side back on track. Felt ""pop"" to left shoulder. Employee did not report accident till Wednesday, October 27th. To clinic Monday November 1st.
While carrying roof straps to a car, employee fell and sprained shoulder. Injury was not the result of a training or compliance issue.
EE lifted a 5 Gal pail of ATF and felt a pop in neck. Employee was seen at medical and was not released for next shift.
Employee was placing safety cones on a MMU trailing cable as it was being moved by an excavator. As the trailing cable was elevated by the excavator, the trailing cable swung freely striking the employee in the left lower leg.
Employee was walking near service truck, one of the doors to a compartment was open and employee did not see it and kneed the corner of door with knee. Small cut on the center of the knee
EMPLOYEE WAS HANGING THE MINER CABLE AND FELT PAIN IN EE'S GROIN AREA.
Operator was trying to pull out a bent roof bolt and EE's right finger got caught between the bolt and plate. Resulting laceration on right finger that end up receiving stitches.
Slipped off or missed step on truck. Rt Arm full of personal items, stepped down and missed a step. Grabbed the grab handle of the truck and pulled right arm.
Employee was preparing to drain oil from swing case of machine. Employee fell approximately 7 feet to ground sustaining multiple injuries.
After crossing belt to update escapeway map, miner stepped in a hole causing a twisted ankle.
Galena Mill - Stairs by break room, employee tripped as employee was coming down stairs, caught self on railing. Went in to clinic on Friday October 22nd, released to full duty. Complained of pain and lack of motion, returned to clinic on October 28th. Taken off work till MRI is done on shoulder - Lost time started on Sunday, October 31st
Employee was exiting the cab of a Skid-steer loader, facing the loader, when left foot caught on a cutting edge bolt of the loader bucket. This caused the employee to trip, twisting left ankle and falling to same level.
RIGHT THUMB GOT CAUGHT BETWEEN A ROCK SLAB AND MACHINE CUTTERS.
Employee alleges they were shutting down plant and kicked the clutch out of gear hitting right elbow on the ladder bolt.
While attempting to move a de-watering pump the employee felt pain in EE's groin. After leaving the mine property EE later sought medical attention at Summersville Regional Medical Center where EE underwent surgery to repair a hernia.
Employee was pulling a piece of belt with vise grips when the belt got caught on something, over extended EE's arm and shoulder causing a pop in EE's left shoulder and pain in EE's arm.
Employee was cleaning garbage out of mantrip and tripped when stepping out and fell causing injury to both arms.

Employee stated was changing a tire on the 488 Diesel scoop. EE was using a slate bar to work the tire loose from the studs. As EE was prying, EE felt a pain in the lower back and groin area. The injury was not reported due to unknown if this is pre existing on medical condition based on employee history.
Employee was walking across the U/G working section when EE stepped in a hole/rut twisting left knee.
Employee was removing bolts on the counterweight on the excavator. When EE removed the last bolt, the counterweight fell (hydraulic ram bracket failed). The counterweight struck the ladder causing the employee to fall & hit their head. EE was taken by ambulance & was diagnosed with a fractured skull.
Employee was cleaning off stone on crusher deck. While bent over shoveling, a piece of stone fell from above and hit EE in the lower back of their head near the top of their neck. This impact caused EE to pass out and fall to the ground.

Was cutting belting with a razor knife, blade slipped and cut through EE's glove & R-index finger.

An employee was changing a valve in a pump. EE was leaning over to pull or pulling up/over the lines and felt a catch in their back. EE went back to work and completed the job and finished the day out. EE called in the next day to report the injury and said was not coming in and going to see the doctor.
Employee was pinning when a half of head fell out of the mine roof and struck them on the right wrist fracturing the wrist.
Employee replacing a leaking o-ring on 5CR Tramping Cylinder bleed valve. EE proceeded to remove the bolt without relieving the pressure. The bolt came loose, it shot out at an extreme velocity throwing the employee's left hand into a nearby conduit. Did not seek medical treatment until 10/22/2021.
Employee alleges they were walking along side of the log feed and they stepped on some rocks twisting left knee.
Employee was operating a dozer to haul waste ore. Employee stepped out of the dozer and a gust of wind blew debris into employee's eye.
Employee was moving the shuttle car cable anchor when EE received an electrical shock. A shard of metal was sticking out of the cable. This was found during the investigation.
EE reports EE was walking in tunnel & stepped on uneven area & noticed lower back pain, later noticed right foot pain. EE went to emergency room, while there EE contacted EE's supervisor to report injury from previous shift who instructed EE to call clinic and report injury. EE off work due to restrictions which cannot be accommodated, seated tasks only.
While waiting at the Mouth of 3 East for traffic to clear up, a tram crew loaded with supplies slid into the bus outby EE causing it to strike the bus EE was operating.
The injured EE got off a mantrip. As EE was walking to tool box located in the crosscut, they felt a pop in knee. EE was unable to put much pressure on it, and was taken outside. Began missing work 10-18-21.
Employee was filling a scrubber tank with a water hose. The pressure of the hose surged and starting whipping around striking employee in the head and causing a laceration.
Was doing maintenance on water pump. Was moving suction line and it had an exposed wire on the bottom side. Punctured hand.
The employee was bolting on the South Mains section. EE was drilling a rib bolt in EE's 3rd row, when a piece of rock measuring 2' wide by 3' long by 6"" thick, fell from the entry appx 9 to 9.5 ft high in between the center t-pad and outer t-pad bouncing off the T-bar and coming down striking EE down EE's right leg and across EE's foot, causing a fracture to EE's right foot
Employee was flipping a stack of roof straps on the supply car. When the stack rolled over, they smashed EE's left middle finger between the straps and the supply car.
EE was standing on drill box to put resin stick in hole. EE slipped catching self with right hand causing labral tear and bicep tendinitis
EE stated that EE drilled EE's 3 ft. hole and went to push EE's stack steel up the hole when the insert broke off causing the steel to miss the hole. The steel hit the top and then bowed out, broke, and hit EE in the right side of the face.
Employee was dragging a belt roller when they felt a burning sensation in their arm.
Employee was helping pull hoses off from a turntable on scoop and was struck by the scoop bill causing injury to foot.
Alleged pebble in boot after working in water (rubber knee high boots on) - unsure of date it occurred. EE is not certain when it happened but it got infected due to diabetic condition and put EE in the hospital for infection, surgery.
Employee was getting off a Rock Truck, jumped down and hurt Right ankle, this happened on 10/14/2021. Employee refused medical attention at this time. Employee waited until 10/21/2021 to go to the Dr. Employee was referred to a Ortho Dr. at this time.
Employee was changing hydraulic hose. After getting hose loose, pressure was on the line and sprayed hydraulic fluid on employee's face and eye.
EE was working at feeder locations on the Mains Section. A rock from the rib dislodged striking EE in Mid Right back.
Employee backed over a rock while operating dozer. No equipment damage. Employee claims they bounced and received a contusion on their shoulder.

Miner was adding second drill steel to set and a finger was pinched between the coupler and the steel in the roof. On 10/22/21, the treating specialist advised miner to stay home to prevent infection to injury.
Employee was changing out top cradle on main line belt and pinched finger between cradle and bedrail causing injury.
Employee stated EE was walking around loader and a piece of rock fell out striking EE in the right shoulder.
IE was picking up a slider tube with another employee when EE felt a pull in EE's left bicep. IE was brought out and taken to be checked out.
Employee was retrieving tools from the belt when a piece of equipment moved and pinched right foot
Employee was greasing equipment at start of shift about daylight. EE moved the grease bucket and stepped backwards off a 1-2 foot drop twisting their right knee and right ankle.
Employee went to remove a remove a broken bag from the incline conveyor belt when they tripped over the wire support and fell on left knee.
Miner was crouched over using a hammer. When miner tried to stand up straight miner was unable.
Employee was throwing over miner cable slack when they felt a pain in right elbow. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was changing a conveyor belt, slipped, and hit hardhat on a concrete pillar causing it to come off. EE was seen for a concussion.
Haul truck operator was improperly loaded causing issue with dumping of material. When material was finally dumped, it caused the cab to jostle.
A fully trained miner was lifting a belt stand when a winder bar fell on foot. The injury was not the result of any compliance issues nor were any protective guards involved.
Shoveling on the belt on the back side of stage loader. When finished shoveling EE crossed the belt at the crossover outby the stage loader and jumped down about 3 feet. When EE's feet hit the ground, EE felt a pain in the right hip and lower back area.
Employee had a piece of drill steel stuck in the chuck. The employee bent the drill steel in an attempt to assist in the removal of the steel. Employee accidentally hit rotation instead of down pressure, the steel spun around and struck the employee on left arm just above the wrist.
Employee was packing bucket of oil up on excavator and twisted left knee
Was opening the fines trailer to empty by turning a hand crank that pulls a chain to open door. Chain became tight and employee pushed harder on the crank and felt EE's back tighten up.
The miner was pushing a rock into the chopper to be chopped and lost control of the rock causing it to fall and land on EE's foot. EE's coworkers helped EE get it off and took EE to the hospital.
Injured went to pick up a Roof jack off of the mine floor to install along the travel way of the M14 and when EE picked up the jack EE felt a pop in EE's forearm. Injured continued working and the following morning had a knot in the forearm of EE's arm. Injured was sent for medical attention and then sent for MRI. For possible bicep muscle tear.
EE WAS DRIVING A GOLF CART TO THE BOTTOM OF THE SHAFT AT THE END OF SHIFT. EE HAD LEG/KNEE HANGING TO THE SIDE OF THE CART. EE WENT THROUGH A CURTAIN AND AN LHD TRACTOR WAS PARKED ON THE OTHER SIDE OF THE CURTAIN. GOLF CART AND EE'S KNEE HIT THE PARKED LHD. HAD A SCRAPE BELOW THE KNEE AND IT SWELLED UP.
I.E. was driving a coal hauler from the face to the feeder. A rock measuring 3.5' x 1.5' x 1-2""Th fell out from the roof. The rock hit the canopy and a piece of the rock came into the cab and hit the I.E.'s right arm. I.E. has a tendon in their bicep pushed back and a specialist took them off work on 10-18-2021 for treatment.
EE lost balance while removing metal sheeting from inside 32B drive house, falling approx. 16' to the concrete floor below.
The employee was stemming holes and while walking EE stepped off in a hole and strained left knee.
Employee was assisting in lifting and sliding tail pulley guard and smashed left ring finger between guard and tail pulley bearing housing.
Employee was walking down the pan line in 8L Longwall Section, when EE slipped. EE felt a pain in EE's left shoulder when EE caught self. Employee reported that the pain has came back over the last couple of weeks. EE worked all scheduled shifts between October 10th 2021 until February 17th 2022 when EE was taken off of work due to the injury to EE's left shoulder.
At approximately 9:30 am on Monday, October 11, 2021, EE was helping other miners clean underneath a conveyor line. EE made contact with the conveyor belt and as a result, appeared to have friction or abrasion burn on their arm. EE refused medical doctor and returned to work. Went to Dr off company time. No Dr report has been given at this time.
Our employee was backing up a skidsteer and was looking over EE's shoulder to inspect area and drove over a rock that jarred EE's back.
The EE was changing the cutter shaft in the L/W shearer. The injured did not ensure that pressure was relieved from the system. As the EE began taking the equipment apart, hot oil sprayed on the EE's wrists and neck. EE was wearing all appropriate PPE. Began missing 10/12/21.
Employee was sitting in a chair at the 100lb. bagging machine when employee swung to the left to catch a new bag that was falling off of the stand, causing employee to strike left elbow.
I.E. was setting up power in a battery barn. When the I.E. setup a breaker for a coal hauler charger on a distribution box they claimed to have received an electrical shock. The I.E. started having medical issues on 10-15-2021 and missed their next shift back to work on 10-17-2021.
EE was helping set shields, they spun the shield and it needed to be pumped up, so they stopped and set the milk to the shield, EE had right hand resting on the shield. When they set the milk to the shield the side seal pin popped out and crushed the ends of EE's middle and ring fingers.
EE was working on a pump in the intake roadway, bent over to pick up a slick line. EE felt a pain in lower back.
Employee setting a beam under the roof mesh, employee pinched hand between rock and roof control supplies.
Operating a scoop heading inby battery first hauling cribbing. Scraped rib hit a rib strap that came into operators cab. Hitting I.E. in the right foot.
A fully trained miner was putting a motor on track when the miner pinched a finger in the jack walker. The injury was not the result of any compliance issues nor were any protective guards involved.
While pulling out a rock in a jam in conveyor #2; bar slipped and hit me on the head. This caused employee to fall and hit shoulder on side of plant (eye beam). At first, thought to be muscle sprain. Employee stated that pain got worse, was sent for an x ray. Found employee had neck injuries (pre-existing). We question whether this happened or not. No one saw employee fall.
Changing conveyor pot on cont. miner, using slate bars to remove CLA disc from pot. Laid a slate bar on the ground after removal of disc. Disc slid off conveyor pan catching end of bar causing it to flip up and strike employee in cheek causing cut requiring 8 stitches.
The employee injured EE's back while dismounting a 992G loader. The injury appeared to be a muscle strain on EE's left side. EE refused initial medical attention but later visited EE's primary care physician when the soreness didn't subside. EE was treated and given a full release.
Employee was working on the Secondary Screen grinding on its rails. Employee advised employee felt something land in eye, but did not think anything of it. Employee advised employee was wearing safety glasses. Employee continued to work from the date of incident (Friday) into Tuesday 10/12/21 before advising what had occurred and that the eye was irritating employee.
While moving a portable staircase, the employee felt a pain in EE's groin while pulling on staircase.
As the employee stepped down off the front of the roof bolter, the employee stepped on a rock causing right ankle to roll. The employee was transported to a hospital where employee was diagnosed with a severe sprain to the right ankle. The employee began missing work on 10/11/2021.
Employee was walking down steps at the float. On the last step, employee was turning left and twisted/injured right ankle while stepping on the concrete floor.
An employee broke both heels when EE jumped to the ground from the deck of a 785B rock hauler. EE jumped because a steering jack hose ruptured onto the turbo causing flames to come up to the outside of the cab area. The EE also received burns to the back of the neck and right arm.
Fueler was lifting a box of supplies using correct ergonomics when the strap on the box broke causing them to jar their back.
During a belt-job, employee felt a pop in shoulder while lifting structure.
We had two shots today. Halfway through the 2nd shot, I was moving the hose over some drill cutting piles and I felt pain in my scrotum. I went to the ER to be examined by a Dr. After receiving an ultrasound and CT scan, I was told I have 2 Inguinal Hernias that will require surgery to fix.
On October 6, 2021 around 11:00 am, the miner started to feel ill, we took EE to the ER and it was later determined that the employee suffered a stroke.
EE was climbing onto D8 Dozer, when EE's knee popped.
EMPLOYEE WAS POWDERING A DRILLED HOLE IN A WORKING ROOM. POWDER HOSE GOT PLUGGED UP. EMPLOYEE WAS HOLDING THE HOSE WHEN THE PLUG BROKE FREE AND IT FORCEFULLY EXITED THE HOLE IN THE WALL. THE MOTION OF THE FORCEFUL EXIT OF THE HOSE OVER EXTENDED (BACKWARDS) THE EMPLOYEES SHOULDER.
EE was loading a roll of belt onto the loader and after fastening the chain, EE stepped off the loader and stepped on a piece of coal and rolled ankle. On October 18, 2021 EE visited a doctor and was given restriction so ankle can heal.
IE was crawling into the shield to set it when EE accidentally hit the pressure bleeding valve on the pressure whip line. The line shot back striking IE in the nose causing a fracture.
A fully trained miner was troubleshooting a rib pinner when EE pinched their fingers with the mast of the drill. The injury was not the result of any compliance issues nor were any protective guards involved.
This incident was just reported to the Safety/HR team as a result of an internal investigation which was held on 11/4/2021. 2 Employees/Miners were involved in ""horseplay"" inside of a designated break area. 1 miner was shoved, fell back and injured a knee.
Shuttle car operator missed a turn, had to reverse direction and backed into the mine rib. EE stated EE struck head and hurt back inside canopy. EE did not change seating position before changing direction. EE was brought to the outside and drove self home.
Employee was swatting at a wasp. In the process the employee was stepping backwards and not watching the steps behind. The employee walked off a step and fell onto their back. The employee sustained a compression fracture to the L1 vertebrae.
Employee was descending ladder of haul truck at end of shift and EE's right foot slipped causing EE to lose balance. EE reached with EE's right hand to grab hold of handrail and felt pain in right shoulder area.
EE stated that EE twisted a knee as EE was turning to walk to the continuous miner on 10-5-21. EE continued to work and consulted a doctor. An MRI was done and EE had surgery on 4-7-22. This accident became a lost time accident on 4-7-22.
Employee was assisting in the loading of 18,000 lb gearbox onto a trailer bed when EE's thumb was pinched between the swinging equipment and the side of the trailer.
Employee got finger pinched between two drill steels. Resulting in stitches.
Employee was roof-bolting on the #3 unit in the #6 entry. When the bolter moved up the employee swung the canopy over toward the rib to install the rib pin and employee's right hand was holding onto the canopy top which caused employee to mash right fingers between the canopy and rib.
IE was removing jack stands from a mantrip using right leg to slide the jack stand out from underneath the mantrip. When doing this, IE stated that knee popped with no immediate pain. Over time IE states that pain has progressively gotten worse causing IE to not be able to bear full weight on right leg. IE was taken off work by a medical doctor on 11/3/2021
IE was driving a shuttle car when they missed a turn and drove into the rib. IE felt a pain in the middle of their back. IE was brought out and taken to the hospital.
The incident occurred in a parking area. The walking surface is paved. At the time of this incident the walking surface was dry, flat, and well lit. There were no safety hazards in the area. The employee's knee gave out with no assistance from any onsite hazards. The employee fell on left knee and was later discovered that left tibia had fractured and required surgery.
A clevis was used to extend chain from monorail to roof. The clevis was not properly secured to the chain causing it to come apart when the work station was advanced out by. The work station dropped approximately 3 feet to the mine floor with personnel in the work station.
IE was operating a roof bolter. A roof bolt was hanging out of the pin tray farther than the other bolts. When EE raised the boom the bolt was flipped up striking EE's right eyebrow causing a laceration. The IE also suffered a minor concussion and was removed from work the following day.
Employee was straightening a roof-bolt with 1 hand when the bolt turned and they felt pain in their neck and right shoulder.
A miner had been stick welding carbon steel in a head box and later experienced welders eye due to exposure to UV light. The miner received medical treatment and was prescribed medication and lost time.
EE was using metal brake to bend a piece of metal in the Morenci fabrication shop. Job was complete and raised the guard to remove material, the guard bled down and pinched EE's hand, EE then panicked and reached over to hit the up button and instead hit the down button causing crushing of fingers.
The injured was attempting to uncouple a draw head between a tram & duster. Employee was lifting the draw head with a slate bar & while lifting the draw head latch with their other hand, they felt a pain in lower back. The employee started missing work on 10/08/2021.
Employee stepped back into a hole and twisted EE's left knee. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee is a loader operator. A tractor trailer drove behind EE and the employees loader came into contact with the belly dump on the tractor trailer. Video available.
EE BOOT CAUGHT IN LAST STEP OF GRATING CAUSING EE TO ROLL ANKLE. EE WAS ISSUED RESTRICTIONS.
A fully trained miner was operating a mantrip when EE hit their head on the mesh on the roof. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was operating a 988K loader feeding a stationary crushing and screening plant. The operator drove the loader over a large rock, adjacent to the muck pile, causing operator to lose control of the loader. This caused injury to the operator's back. The operator required off site medical treatment and time off per medical professional
Took a big step out of skytrack and knee popped.
On Saturday October 30, at 10:00 am a miner was struck in the head and neck area with a Cat 324E long reach excavator bucket causing an injury to EE's neck. EE was able to return to work with restricted duties on December 15, 2021.
Employee had been loading dirt from stockpile that is hauled in from offsite. Employee got out of machine to check cleanliness of bucket. Employee was climbing steps to enter cab of wheel loader when hand slipped from hand railing. Employee fell backward landing on ground. Employee sustained broken ribs from fall.
EE was bending a 6' bolt. When EE was pushing on the bolt, the bolt shifted and the employee fell forward and strained right shoulder. EE was working regular duties until seeing doctor on 12/28/21 and being placed on light duty. Surgery scheduled for February 2022.
EE was rolling out a torch line from EE's service truck and turned to pull some more line and twisted EE's right knee while turning back toward the truck.
Ee was operating a 777 haul truck, getting loaded by a loader. Ee reported the loader operator dropped boom of loader on side of truck, more than once, causing the truck to come off the ground, jostling EE in the cab. Reported to supervisor on date of incident. Seen by doctor on 10/29, put off work pending follow-up.
A haul truck operator twisted knee while stepping off van at 43 tie down at start of shift. The event was reported to HS3 and operator stated they did not need further evaluation. The employee states they began to feel increasing discomfort on the knee and reported to a clinic in San Carlos.
An employee's finger was pinched resulting in an avulsion that required a skin graft surgery.
Driller was fighting a bad hole, mistakenly got hand under bottom stop for drill top head where EE knows not to put them, top head smashed right thumb.
Stone fractured when splitting
Ee dismounting shovel using stairs. Wearing large overboots due to muddy conditions, tripped over large rocks in shovel track grooves, landed on left knee. Follow-up with orthopedist on 1/25/22 resulted in referral to physical therapy making this reportable for treatment. Ee working full duty since date of injury.
Lift of structural beam being made. Miner was inside safe zone upon commencement. Load began to pivot as it came up. Miner placed self in pinch point in an attempt to stop collision between load and separate piece. Load impacted miner and piece at same time.
Employee tripped over a hose and fell into a sitting position.
An employee was helping offside and was moving bolts to the bolter and dropped the bundle on their left foot.
Squatted down to pick up a tool, felt pain in left knee
Tripped on hose in process plant resulting in a sprained ankle
MOVING ROCK, ROCK LANDED ON RIGHT RING FINGER
Forklift operator dismounted forklift, moved an empty plastic tote, and strained left shoulder. Medication was prescribed at primary medical facility, awaiting MRI for further evaluation.
Employee climbed a 6' ladder to clean the add water sensor. As EE was descending the ladder, EE lost footing causing EE to fall and strike their side and back on the concrete pad. The employee declined medical attention at this time.
After completing a mucking cycle IE was waiting on drill basket to be sent in. IE was standing at shaft bottom when a rock loosened behind the shaft liner and fell from approximately 33' striking IE in the back of the head, neck, and shoulders. The loose rock come from fire clay that was saturated with water causing a piece of black shale approximately 18""x6""x6"" to fall.
EE stepped onto a rocky surface and rolled EE's right ankle causing a fracture.
An employee's finger was contacted causing a fracture.
On 10/20/21 IE was walking the slope belt during an on-shift examination. IE stated IE stepped down off a manway door (approximately 4' - 5') and IE's knee buckled resulting in pain. The IE continued to work, on 12/29/2021 IE began receiving a physical therapy that per the part 50 guidelines makes this incident reportable.
The employee was pulling and pushing on 8 and 12-inch pipe and valves to line up the pipe for fusing. On 10/21/21, the employee reported swelling and discomfort in the lower abdomen. The employee was transported to BMGH diagnosed with a small hernia. On 10/27, the employee requested a follow-up at the clinic.
The miner operator stopped mining, swung the tail toward the right rib and exited the cab. It is unclear why EE exited the cab of the miner. EE was a couple feet from the operator's compartment when the arch fell, striking EE in the head/neck/shoulder/upper back area. The injured employee was provided first aid in the mine and taken to the Memorial Hospital of Sweetwater County.
Loadout operator was on top of a box truck closing hatches. The top of the box had a thin layer of sand causing the EE to slip and stumble backwards, tripping over the small rail on the edge of the box causing EE to fall over the side and between the the 2 boxes on the trailer and hit the bridge of nose on the second box which was directly behind EE.
While operating the drill press in the wash maintenance shop to make a hole on plate, the drill bit got stuck and the base plate jerked and turned hitting the associates hand and cutting middle finger on left hand. The associate had to get sutures.
There were heavy rains all morning while we were working on equipment. After completing work on the D10R I was checking the fluid levels. When climbing off the dozer my foot missed the step on the push arm and began to slide. My feet went above my head and I fell 4-5 feet on my back.
A miner operating a 472 haul truck (CAT 789 model) was exiting the cab, the operator's heel hooked on the door seal causing EE to fall backwards, hitting left elbow and forearm on radiator. EE was taken to the local hospital for further evaluation; where it was found that EE sustained a fracture to the left Ulna proximal to the elbow.
Stone became stuck on roller table for the hydra splitter. The employee had the skid steer lift the rock from one edge. The skid steer lifted one edge and stopped moving. The employee then pushed on the rock causing it to fall on EE's wrist.
Employee was stepping into man van at beginning of shift when employee felt knee pop. Upon exiting the van, employee felt pain and was unable to put weight on left leg.
Employee stated that EE was getting off of dozer. The push beam was up on jacks. As EE stepped down off of push beam step, EE felt EE's right knee buckle when EE stepped onto the ground causing pain in EE's right knee aggravating a previous injury.
Employee was operating a shuttle car when a piece of rock was thrown from the continuous miner boom and struck employee on face and forehead.
EE was walking down a set of stairs and turned right at the bottom landing of the stairs and felt a pain in their right knee.
Ee reports moving a loader tire and felt pain in their shoulder. Seen at Golden Valley, diagnosed with strained right shoulder, released with restrictions site can accommodate.
A chunk of coal slid down the ranging arm on the shearer, then fell off onto the walkway. Some pieces struck the employee on the top of EE's right foot, resulting in a fractured toe.
Employee stated was getting off of a dozer. As EE stepped down off of push beam step, EE's right foot stepped on a rock causing EE to twist right knee. Went to doctor to get checked thus incurring lost time.
Employee was cleaning top of rail car with jackhammer when EE felt a tweak in their back. Finished shift and went home as back was not hurting at that time. Was feeling some tightness, employee was sent for an evaluation and waiting to see doctor.
Employee was walking around electrical sub conducting checks when they rolled their ankle when a rock moved under their footing resulting in a chipped bone in the ankle.
Employee was under the crusher pulling on wrench while removing the lube line - when they felt a burning in their stomach.
While adding a steel the steel fell out of the top of the loader. The operator thought they were able to get the steel back into the loader properly. The operator then threaded onto the steel to make the next connection. The steel came free from the rotary head & dropped approx. 2ft onto the drill table & then slid off the table & dropped 3-4ft striking the operator's ankle/foot.
EE was riding in a buddy seat of a haul truck and felt pain in the upper back. EE was taken for medical attention at that time. EE remains on restricted duty.
Injured fell and hit elbow on the coal rib and the mine floor causing elbow to become dislocated. Injured was transported for medical attention and and elbow was put back in place.
Employee reported they were swinging a large sledge hammer while installing the gabion plate back in the grizzly. In order to do that they had to swing a large (heavy) sledge hammer to beat rocks out. *Employee did not report injury until 11/29/2021*
Inj. had knocked stopping with the scoop & was removing the material from the crosscut. A piece of 3"" pvc water line was lying in the gob unknown to operator. While cleaning the area the water line became entangled between the rib & scoop wheel. When the waterline came loose struck inj in left side of face resulting in a laceration to the lip & fracture to the eye socket.
Employee was replacing a motor on coarse screen feeder. The winch holding motor slipped slightly, and the motor hit the employee's arm.
EE WAS TIGHTENING THE BOWL ON EE'S 4' CONE CRUSHER AND THE CABLE EE WAS USING BROKE STRIKING THE EE IN THE LEFT FOREARM CAUSING A FRACTURE AND LACERATION.
EE was climbing boarding ladder for grader on ready line, placed foot on first step, grabbed handholds, began to pull themselves up on clean, dry step from the ground & felt pain in right shoulder. Diagnosed right shoulder tendinitis.
Employee experienced rough loading event. Diagnosed with low back strain and continued full duty. A few weeks later on 11/1/2021 the employee went to the doctor and requested time off because back still hurt. Employee is off duty until employee follows up with a specialist out of town.
While completing maintenance on crusher EE got fingers pinched on the left hand.
Injured was swinging sledge hammer while installing bit inserts for the miner. Injured said the next morning EE had trouble lifting EE's arm. On 10/11/21 injured filled out accident report and was set up for appointment to be seen for shoulder and on 10/13/21 injured was placed on restrictions.
HME operator was mounting a 988G FEL at the start of shift. EE bumped left knee on the bottom 2 step ladder mounting bracket (normal bracket). EE reported knee tightened up 5 hours into the shift. First aid/home care measures were followed until employee stated they could no longer walk on that leg 2 weeks later. The determination for work-relatedness was made on 11/29/21.
A mill operator trainee was walking on grating and tripped while maneuvering around a pillar. Their foot caught the pillar and the employee fell to foot level catching themselves with their hands. The employee then began to feel pain in their thumb and wrists and numbness in their fingers.
Lab tech lifted bucket (approx. 20 lbs.) about 36 inches over a rail to dump into a dumpster felt a pop. EE refused medical care at that time, iced and monitored it. Next day EE stated it felt better. On 10-26-21 EE stated it wasn't getting better and wanted to see a Dr. EE was given restriction and Tylenol. On 11-24-21 MRI was performed. Surgery was scheduled for 12-16-21.
Employee got out of buggy and when EE stepped out EE sprained EE's ankle
EE was crossing under/over a railroad car when EE originally felt a strain in the lower back on Monday 10/4/21. It wasn't until Saturday 10/9/21 the EE experienced pain bad enough to go to the hospital.
Heavy Equipment Mechanic was welding a small crack by the mounting bracket of the 980M wheel loader's fuel tank. A flash of fire occurred and mechanic received burns to face, chest, and left arm.
Employee was walking on a catwalk when they slipped and grabbed onto the handrail for support. In doing so, a pain was felt in the shoulder. Injured went to doctor on 10/7/21 and was taken off work.
Removing the belt wiper on the conveyor feeding the gravel plant screen. While removing the 2nd bolt holding the wiper, the wiper fell down and pinned employee's hand by pinching it between the wiper and frame.
An employee was on knees reaching into a box attached to the FK pump at finish mill 2 to remove a flapper arm. When employee lifted the flapper arm employee felt a sudden pain in lower back. The employee was issued a work restriction and prescription medication.
Employee was assisting mechanical maintenance replace belt on #3 palletizer bag flattener. While feeding the belt through the rollers, the employee felt a pop in the left shoulder. Employee later went to urgent care and was placed on restricted duty, cannot lift more than 3 pounds with left arm or 10 pounds combined arms and no ladder work.
While cleaning material around the access hatch on C1 Conveyor the employee lifted the hatch and twisted their back. Employee felt pain in their back but did not report it to management until 11/1/2021. Employee was taken to occupational medicine for evaluation and was later released back to work with restrictions.
Employee complained of neck and back pain after receiving the first dipper of material from Shovel 6.
When the team member stepped off the last rung onto the ground the team member planted foot wrong and rolled ankle.
7am n 11am on 10 29 21 EE W was working with EE J performing maint. on an excavator bucket, pin wgt 80# EE W decided to lift the pin n not use equipment provided. Witness warned EE of weight but decided to lift anyway, while lifting EE twisted, heard a pop. EE kept working then later reported to lead. EE taken to med facility, lumbar strain, given medication and work restriction
Employee was rodding out Mill #1 with a pry bar when they smashed their left index finger between the pry bar and an I-beam behind them.
Slip/trip accident. Carrying rebar and tripped causing them to fall. Employee put their arm out to catch themselves which hit the rebar matt.
While being loaded by the shovel a boulder fell off and hit the dove tail of the haul truck. Operator began to feel pain in lower back.
Employee was stepping over an i-beam on the ground, when the toe of the boot knocked the i-beam. The employee lifted their leg up and landed on their foot really hard. The employee experienced pain in their knee. The following day the employee sought medical treatment and was placed on restricted work.
Stepping down from edge of VSI, they slipped and rolled R-ankle
Operator was descending down a ramp with a fully loaded Bulk truck at about 15 miles per hour following a water truck. Operator had both the retarder and jack brakes applied but started to slide and over corrected and slowly turned the truck over.
While employee was guzzling by 18-19 conveyor EE stepped on a 1 1/2 inch rock and rolled ankle.
Employee was bending a roof bolt and slipped causing it to hit them in the right bicep, resulting in a bruise on the arm and they worked shorter hours for two days.
Maintenance Technician sustained a laceration to the left hand fingers from sharp metal.
While getting off the fuel truck, EE slipped on the step while hanging onto the bars and pulled something in the left shoulder and lower back.
Injured employee was pulling a water hose while cleaning up an area at the wet plant. While walking EE stepped on a sand pile causing EE to trip injuring EE's right knee and back. Employee was diagnosed with a sprained right knee and back and was given work restrictions.
As the employee was approaching a manbus that was parked on the 3 East Trackway directly across from 9 Vent Bottom the employee tripped and fell forward.
Ee was reaching to put something away on bus when bus started to move. Ee started to fall and EE caught self from falling, straining their shoulder. Seen at Golden Health, diagnosed with a shoulder strain, and released with restrictions that site could accommodate.
Employee was getting onto a platform when EE's hand slipped, causing EE to fall back and land on a gang box, causing injury to EE's lower back and wrist.
Employee was operating haul truck that was being loaded by shovel, when loaded felt pain in neck and lower back.
Miner was removing a slusher block from an eye pin at the face and felt something ""give out"" in left shoulder. Miner was diagnosed with a muscle strain of the left rotator cuff at the shoulder, and a sprain of the left AC joint. Cleared to return to work with restricted duty (no lifting above shoulder height with left arm). Recommended Physical Therapy evaluation and treatment.
EE had completed a belt splice on the surge tunnel belt, and was applying a strap to the old belt to remove it from the conveyor. EE was descending from the work area, left pant leg caught a piece of rebar, causing EE to lose balance and stumble. Subsequently, the rebar caused a puncture to the employee's left calf. EE was given a prescription and placed on restrictions.
While walking to their vehicle the employee rolled their ankle.
Employee parked front end loader and was getting down off the ladder. When EE took the last step, the ground was dug down further than EE was expecting and EE fell on right shoulder. Employee bruised their shoulder and worked light duty the next shift.

Employee walked under a low clearance crossing, after clearing the lowest point they stood up and bumped their head on the return idler guard.
Lifting a piece of pipe (8 inches round, 5 foot long) up to the top deck on the 7th floor of the preheat tower to be installed. Thinks they strained right elbow lifting pipe.
EE tripped over step in walkway causing laceration to left knee. Laceration required stitches to close.
Employee was getting loaded at Shovel 6 with left arm resting on the window. Resulting in shoulder pain.
Employee was walking down steps when EE felt pain in the right knee.
Employee was shoveling rock on 10/20, while shoveling employee felt a small pop on their wrist. Employee did not think much due to regularly popping joints. The morning after employee felt discomfort on their arm. While conducting similar work today, employee experienced pain when working. Employee then reported it to their supervisor, and was evaluated.
Employee was repairing ditch drain grating when foot fell into ditch injuring rt. knee. Employee continued to work until 11-23-21 where employee had surgery on rt. knee
Individual was in man lift with harness on and tied off was loading totes of collector in main tank. When reaching through hand rail on man lift EE felt a pop in left bicep/Shoulder. Individual went to the Doctor this morning and was diagnosed with most likely a pulled bicep tendon. 1 week of rest with pain meds if pain persists after 1 week than X-Rays and further Diagnoses.
Operator had climbed into the seat of the loader and went to shut the door. As EE pulled hard on the handle, EE experienced a sharp pain from the left elbow to the hand. Operator was swapping loaders with another employee.
Emp. was on the bottom floor of the KMAG Screening Plant planning clean up activities when EE was struck on the left shoulder/neck by a piece of product buildup falling from above. Emp. taken to ER and placed on restrictions and given prescription meds.
While installing delineators on the new B-line ramp, an operator sprained an ankle on the uneven ground by the berm. Operator did not see a rock and stepped on it which sprained the ankle.
Maintenance employee was carrying a head shaft sprocket down a flight of stairs and momentarily lost balance. While regaining balance, the employee's right ring finger was caught between handrail and sprocket, resulting in fracture of finger tip and stitches to close wound.
Employee was driving a ground rod into the dirt using an electric hammer. The hammer slipped off the tip of the rod causing employee to fall forward. Employee fell into the ditch and landed wrong on foot.
Employee part of maintenance crew removing analyzer probe housing from back of kiln. While working to guide probe and rigging around obstacles, rigging slipped causing weight to shift and end of probe housing to fall striking employee on instep of left foot behind safety toe of boot causing foot to be broken.
Attempting to free a piece of Plug gear and EE hit finger with a sledge hammer and cut finger on the stone.
Employee was climbing ladder onto haul truck, hands slipped, twisted left side back.
Employee rubbed up against hand rail as they were picking up screen.
Using a pry bar inserted into the notched holes of the barrel of a capstan fitting to open a gate. Employee applied force with the pry bar it bent and rotated causing the employee to lose their balance and fall onto the lime loading grating. During the fall EE's right arm braced the fall. EE's arm struck the grating and slid into the opening causing a fracture to the right ulnar.
Injured was cleaning out under some conveyor belts using a water hose when water deflected onto injured's face an into right eye. Injured was wearing safety glasses at the time. Injured went to Dr. on 10/19/21, diagnosed with a corneal abrasion.
Offloading at 4500, the hose was leaking so EE lifted the hose to adjust a jack stand and pulled a muscle in EE's forearm.
EE was using a water hose to wash out EE's haul truck. As EE was walking backwards without looking, EE tripped on a cement k-rail and fell over it, in the dirt on EE's back. EE did not report the injury until 10/18/21
Haul truck operator backed into shovel dipper at 0.5mph
Employee was in the process of removing adapters off of the 368 bucket when a J- hook they were using stretched out and came off the adapter striking the employee in the left forearm.
Was putting a conveyor chain in and it got stuck. When EE got it unstuck it pulled EE's left hand into the conveyor cutting the tip of EE's index finger.
Employee was trying to free blockage of rock/material in a tail pulley using a pry bar. The pry bar slipped and the employee fractured EE's right ring finger. Equipment was de-energized and locked out when employee performed this task.
While stepping on the skid loader emp was power washing, emp slipped and fell (about 18"") to the ground injuring rt shoulder
Employee was using a shear in the maintenance shop to cut a piece of metal, during the task the metal shifted causing EE to lacerate EE's index finger. The employee was seen by a physician and treated for a laceration, receiving stitches.
Leg slid on some metal grate causing an approx 10CM wound requiring stitches.
Mechanic was using a sledge hammer to get a pin out of a thumb cylinder and felt a pain in stomach.
While moving a lift chain, the pip it was wrapped around rolled pulling the hook through the left thumb
EE slipped on stairs and hurt back. Incident did not become reportable until 10/18/21 when EE was given restricted duty by Doctor.
Employee had been sanding motor shafts, turned to walk away and tripped over a saw horse and fell onto hands. Fractured Left Wrist.
Employee claims slipped walking down steps and injured back.
Employee was driving a lube truck in a tramway underground. They hit a bump causing the top of their hard hat to hit the FOPS of the cab. This caused the employee to have neck pain. The employee was taken to the clinic and diagnosed with a cervical strain.
Lifting end cuts from the radius line and turned to throw a piece into a grout box and injured EE's back
An employee was changing valves at the loadout silos. When the person put a pipe into the fitting to move the valve, the bar slipped and the employee felt a pop in their shoulder. The employee was placed on restricted duty 6 days later.
Employee was working on changing belts on 68 pump in the screen house. EE bent down to help guide the belt onto the sheave and that is when EE felt a pop behind right knee, EE then grabbed partner so EE didn't fall to the ground.
While unloading 5 gal. water jugs from a flatbed truck EE felt pain in EE's right shoulder while lowering jugs. On 10/15/21 EE saw a physician, was diagnosed with right shoulder/elbow strain. Restricted duty (5 lbs. lifting max) and Physical Therapy Evaluation were prescribed. On 10/22/21 the physician increased wt. limit to 15 lbs., and recommended Physical Therapy Treatment.
EE dismounting/exiting cab of excavator missed the hand grabs and step falling 2-3 ft. to the ground. EE was holding items in both hands while exiting cab.
Injured claims hit head on the canopy of the mantrip as the ride was coming out of the mine. Injured reported the incident the following day. Injured was sent for observation and diagnosed with Myofascial strain and placed on restrictions.
Employee was exiting Skid Steer Loader. Misstepped and came down hard on heel of right foot. Sprained ankle and deep tissue bruise to right foot.
Employee was tramming JLG when they engaged a pothole full of water. As the tire dipped the energy transferred to the bucket and caused employee to bounce up and when they came down they twisted their right ankle.
EE reported to management on 10-12-21 that during their break they found a bump on their body. EE said they are not in pain but felt the bump and was concerned as they did not recall one there prior to 10-12-21. On 10-21-21 EE was diagnosed with an inguinal hernia.
EE was using a winch (model 492) to back off the gravity take up weight when the winch handle slipped out of EE's hand while changing hands. The winch handle then spun around and the handle hit just above EE's right wrist. The handle was free spinning once EE's hand slipped off of it. Right wrist has a fracture of the ulna bone. Injury not in a cast but splint.
Employee was working on mobile equipment and the engine compartment door fell on the employee's left hand
Employee states that while adding dry ice left foot slipped into a hole causing employee to do the splits. Employee complains of pain in the groin area and right lower abdomen.
Lifting a box of core samples into a trailer and turned wrong and injured lower back
EE was removing barricade on a slight incline when EE slipped backwards. As EE was falling, EE placed hand behind to catch self. In doing this EE injured their finger on rocks in the area.
WHILE WORKING IN MILL MAINT. SHOP EE WENT TO REMOVE A PIECE OF METAL SHAVING FROM THE VALVE AND IT CUT THROUGH EE'S GLOVE. EE SUSTAINED A CUT ON EE'S RIGHT HAND 4TH (RING) FINGER.
While shortening the rope on the #2 hoist, the thimble assembly pin was removed and two employees pulled on the rope to remove it from the bracket. When the thimble was free of the bracket, the rope twisted, the thimble swung and struck the miner in the face (hitting miner's hardhat and safety glasses). The miner received lacerations around eye and was taken to urgent care.
Employee was at the middle of the bolter when the bolter moved up catching the tip of their steel toe boot under the tire resulting in the left foot being sore. Employee worked two days of shorter shifts.
While troubleshooting a launder level transmitter, the employee tripped and injured knee. Employee continued working, and at end of shift asked for medical attention. Employee was taken to the clinic and given restrictions. Currently ONGOING.
EE was lifting a wheelbarrow by its handles when EE heard and felt a pop in lower back.
Employee stepped over a hose on level ground, and felt ankle twist as they did. The employee continued working for an hour, and foot began to swell. It was discovered that they had a broken bone in their foot.
Moving a water pump over a mound of dirt with another employee and awkwardly moved and strained back.
Employee was positioning himself to clean off a loading spout screen at silo17. The employee tweaked their back when EE was moving between the air slide and the handrail. EE left shift but later reported that their back was tweaked. EE later reported on 10/13/21 that their back was hurting. Then saw saw a doctor.
Employee was air lancing along the south wall of stage 1 to remove build-up of material. As the employee pushed forward with the air lance, they felt a pop in their left shoulder.
While wrenching on a cylinder on a piece of equipment the wrenches pressed together and cut into the nail and tip of the mechanic's rt ring finger requiring stitches.
WHILE WALKING FROM EMPLOYEE PARKING LOT TO EQUIPMENT IN THE DARK, THEY SLIPPED AND FELL TWISTING RIGHT KNEE.
Employee got dust in EE's eyes while cleaning up a pile of spilled material. The employee was prescribed medication for EE's eyes when sent for medical evaluation.
The employees were working on a 150 lbs. head tank frame that was sitting on the walkway grating. As they went to move the frame, EE #1 noticed welding rods and attempted to move them as EE #2 moved the frame and pinched EE #1's middle finger, right hand, between the frame and the grating. As a result the fleshy end of the finger was cut requiring stitches.
An operator and trainee on Haul Truck 22 were rough loaded at Shovel 4. The operator proceeded to dump load, tied down and contacted supervisor to report event. This resulted in the operator to experience lower back pain.
Replacing a tail pulley in a screen feed belt, the new pulley assembly was suspended in place when the maintenance employee was using emery cloth to smooth the shaft. The pulley assembly shifted, trapping employee's thumb between the shaft and the structure. The crush injury resulted in a tuft fracture of the right thumb, and loss of the thumb nail.
Employee drove D-10 Dozer over an embankment and dozer fell on its right side. Employee cut right elbow and punctured right shoulder. Both required stitches
An SX Employee was re-routing the flow to the holding tank on the G-train for skimming organic. The employee explained that while walking on top of a 10-12"" stainless pipe to cross to another pipe their right foot slipped causing them to straddle pipe and injure lower back. Incident was upgraded to a Restricted Duty on 10/07/2021
Tech was using a wire wheel, a bristle came loose and penetrated the skin in the thigh area on the left leg. Tech went to the hospital to have the piece removed.
2 mechanics were reinstalling a plant crusher accumulator. The part had 3 sheared bolts and broken seal causing a hyd oil leak. EE was lifting the part back into position by hand while waiting for the hyd jack to hold it in place. Accumulator got in a bind with the support clamp and fell back to the deck smashing EE's left middle finger. EE was wearing gloves. Part weighs 30-40lbs.
EE was washing down conveyor and the end of the water hose separated from nozzle striking EE's right hand by their thumb.
Employee was taking a vac hose off a storage hook. When the hose straightened out it caused EE to fall and hit EE's elbow on the floor. After the shift the employee realized the fall resulted in a cut to EE's right elbow which was treated at clinic with glue.
IE was walking belts with a co-worker as part of the underground training program. While working on the opposite side of the bottom belt and EE's co-worker, the IE had turned EE's head to watch the belt, striking EE's left ear with a piece of hog panel/wire mesh. The IE was and taken to the emergency medical facility and received nine sutures to close up the laceration
While backing into spotted shovel bucket, the tail of production truck made contact with shovel bucket. The operator reported discomfort in their neck and was transported to the clinic for evaluation. After evaluation, the operator was prescribed physical therapy for a neck strain.
Employee was attempting to disconnect two unloading hoses, which were stuck. Employee forced hoses apart, and one end swung up and struck employee above upper lip, causing a very small cut. Employee suffered a small cut on inner lip when contacting teeth. The inner lip cut became infected several days later and required antibiotic treatment.
EE was removing a guard when the guard slipped pinching the EE hand against a guardrail causing lacerations.
Emp was trying to open a dump chute using the slide hammer. As EE was sliding the hammer, the handle broke and EE's thumb slipped between 2 pieces of steel, pinching it. EE was taken for treatment and received sutures. EE was released full duty and returned to work.
Employee was walking across a bridge when EE slipped and fell striking their elbow.
Employee stated while EE was trouble shooting a 4-wheeler control panel, EE was using a utility knife to cut a zip tie that was holding the leads together. EE was holding the leads with the left hand and as EE was cutting the zip tie, the utility knife slipped causing a laceration between EE's left index finger and thumb.
Employee was cutting a piece of rubber, in screen 104, and cut leg with a razor knife. Employee received two stitches.
Two employees were cutting open a tote from each side so they could empty the product out of it. They were cutting towards each other without realizing it. As the one pulled the knife out of the tote it nicked the top of the other's left hand. IE received 3 stitches. IE was able to return to work immediately and has no work restrictions from the Dr.
Employee was moving a screen when EE lost balance. As EE lost balance EE began to drop the screen and the corner of the screen made contact with EE's right shoulder.
While cutting 1 1/4"" pipe using a wheel grinder, the grinder slipped, striking the employee above the knee resulting in a laceration.
While doing a sound and vibration test before drilling a roof bolt hole, a softball size piece of material fell striking employee in the mouth. When employee went to the Dentist on December 8th, a tooth was broken and needed repair.
Pinched finger when fuel hose slipped.
EE was removing the impellor off of a water pump. The impellor fell off of the shaft and onto EE's right arm. It cut EE's forearm open leaving an approximately 3"" long wound.
Injured states ""I forgot my knife so blaster was throwing their closed knife at pillar next to me which hit and opened on impact then bounce (sic) and stabbed (sic) me in the arm."" 3 stitches required.
An employee was assisting in re-welding a section of the mainframe liner, during the process, employee was exposed to arc-flash.
Employee was rolling rock on the splitter and the rock fell off the side of the splitter and hit EE's foot.
Operator tripped on sidewalk at the Ironclad falling and hitting face on handrail. Dry conditions and was at the end of shift. Employee received sutures for laceration.
Employee was welding inside screen, when the wind blew something into left eye. Doctor removed a small sliver of metal from the eye and prescribed antibiotic drops. No time missed or work restrictions.
Employee was using a lifting jack to align the low structure to insert a pin. The jack slipped causing the pin to pinch employees' thumb between the pin and low structure. Sutures were required to repair the wound.
EE's description, 'While cleaning up from a job at 4:50pm in Alesa 1 building I was putting away my tools. While trying to remove a bit from my drill. My glove became caught on the drill and spun my glove with my fingers backwards. This resulted in spraining my ring finger on my left hand. Gloves where brand new from issue less than an hour old.'
During the installation of rebar for the surge tunnel, a piece of #10 rebar was being placed and dropped. The bar was being installed by several men on a wall. The bar that fell was about 51'-0 long. It was being tied horizontally. When they were handing the bar across it slipped out and fell, landing on EE's wrist.
2 employees were removing a set of large bolts with a 3/4"" impact and a backup wrench. The employee holding the backup allowed a hand to slip and had left index finger smashed against the equipment, resulting in 6 stiches. Medical care was handled quickly and without delay.
Debris fell off face shield causing eye irritation.
Miner was rolling crusher rotor when left hand pinky and ring finger got caught between crusher rotor and side wall of crusher housing.
Employee was pulling slack out of the belt with a chain, the chain slipped loose and struck EE on the face and left eye area. 3 Sutures were required to repair the wound.
EE was exiting surge tunnel and took off hard hat to scratch head and while turning, struck a piece of metal located above, causing a small laceration requiring 4 stitches to close.
Troubleshooting a hydraulic clamping system on a Symons 5-1/2 Cone Crusher. The pump had been run out of oil due to a leak. The pump was off, at this time they removed three lines inside of the unit to release air. The lines were not properly installed, resulting in bypassing of the relief valves. This resulted in lacerations to EE's right hand.
Employee was tasked to test a High voltage cable. While preparing the cable to be tested, employee stepped down on a slope to get a piece of wire, losing footing and spraining left ankle. Incident became reportable 11/1/2021.
Miner was walking by roof bolter and struck hand against the machine striking a burred edge. Resulted in a laceration required four stitches.
Trimming off black expansion material with utility knife and using the knife in an improper manner cutting towards self. Cut through the glove causing a laceration on EE's left hand.
EE was working monorail slack using the push/pull unit. EE rested hand on top of the monorail and the push/pull dolly pinched right middle finger between the monorail and the monorail dolly wheel.
Shift electrician was removing a rusted connector from a light fixture. Pump pliers slipped from fitting. EE's left thumb struck light reflector resulting in 2 inch cut. EE received stitches to close the open cut
Employee was exiting a skid loader and used the top edge of the bucket for foot placement while climbing down. Right foot slipped and caused fall forward resulting in the area just above the knee to come into contact with the edge of the skid loader bucket and foot slipped backwards off the bucket causing overextension of the knee. Recordable 11/26/2021.
Worker was handling panel at grade on work benches. Was attempting to knock snow and ice off a panel by repeatedly picking up and dropping it on the work bench. The wind caught the panel and worker tried to dump it off the bench. It twisted and one corner popped up and contacted cheek, resulting in a minor laceration requiring sutures.
Employee stated they were shoveling calcium and felt their back tighten up
IE was attempting to cut a lifeline cone with a razor knife. The knife slipped off the life line cone and stuck into IE's left hand causing a laceration that required 4 stiches to close the wound.
The employee was working in the warehouse rigging yard putting pins on a pallet. When EE rolled the pin into place, the pin moved faster than expected and smashed EE's left thumb in between another rigging pin.
Underground blaster was loading a second row of holes on the face and as EE reached up to the hole EE's foot slipped causing EE to fall into the face and cut right forearm on a rock protruding from the wall.
Injured employee had removed a screw for repair of equipment. Employee was reinstalling the screw when the screw drill was pressed and the screw had a rough metal edge and caught EE's index finger. Cutting it 3 inches in length requiring stitches. No loss of work and was able to return the following day.
Cutting threads off a wedge bolt jaw crusher with a cut off wheel on a grinder; EE was wearing glasses and a shield but metal was in left eye. Employee did not request medical treatment until 10/22/21
The miner III trainee was utilizing a jackleg to install a low bolt. The trainee was applying pressure to the cob to keep the leg from pushing the machine up. The crows foot of the jackleg kicked out causing the machine to fall pinching the trainee's index finger between the cob and the leg. The incident resulted in a laceration requiring sutures to close, full release issued.
Miner was getting a waste fuels sample and was bit by a spider on the back of head/neck.
Employee was operating the head gate drum of the shear in 5B LW when a rock came off the drum and hit the employee on left hand. Employee received 3 stitches to left index finger.
On 10/19/21, employee hammered a loader tooth pin. Metal came off, struck the employee's cheek, causing a cut. First aid was provided. Injury became reportable 11/11/21 when employee requested medical attention for prolonged irritation in the wound. Foreign metal body was removed and sutures were applied.
Employee was changing silo hammer line valves when EE experienced a sharp pain in EE's Left shoulder. Employee reported the incident but did not seek medical treatment at that time. Employee tried home self-care but the shoulder did not get better. Employee sought medical treatment and was referred to physical therapy, making it a reportable injury.
Employee was changing a rotation hose on the 419 roof bolter and the channel locks employee was using slipped off the fitting causing employee's right palm to come in contact with a metal tag on the bolt tray. This caused a 5 inch cut that required 5 stitches.
EE working on ladder was cutting corner of guard on finisher in Concentrator to fit around newly installed pipe. While utilizing grinder w/cut-off wheel, grinder kicked back contacting left knee causing laceration.
Miner was struck in the face by a shovel full of rocks while hand cleaning area
Employee was struck in the left ear while advancing trunk lines. Causing a laceration which resulted in 13 stitches. The employee returned to work next shift with no lost time.
The Employee was in #9 entry on the # 3 section roof bolting. While trying to maneuver the drill steel into the hole in the roof mat a piece of rock fell striking EE's rights hand. The area was approximately 6.5 feet high and damp.
The Utility supervisor stated that while operating the Telehandler it felt like a piece of dust went into the eye.
Screen belt bearings went out. Shut down crusher. Cleaning return rollers on belt with hammer. Under screen belt caught hammer and pulled hammer and Rt arm into the belt. Laceration on Rt hand, friction burn on forearm.
The employee was changing screens in the wash deck. The screen slid down cutting the wrist of employee.
Employee stated employee was operating a scoop near spad #2683 on the intake airway when a cable bolt that was buried with mud was flipped up striking the employee's left ear. Employee continued working the rest of the shift and did not receive medical attention until 10/17/2021.
Employee was using a sledge hammer on a wedge. A portion of the hammer sheared and struck EE in the forehead causing a cut.
Employee was using hammer jack for 5 1/2 hrs to clean out material from 1700 even feeder and experienced back pain the next day.
WAS LIFTING UP ANCHORS WITH A HAND WINCH AND EE THOUGHT THE WINCH WAS LOCKING AS IT WAS RAISED. WHEN I RELEASED THE HANDLE IT SPUN AROUND AND HIT EE'S RIGHT THUMB. UPON XRAY IT WAS DISCOVERED THE TIP OF EE'S RIGHT THUMB WAS BROKEN. A SPLINT WAS APPLIED.
Employee was cutting roller path when pant leg caught fire. Employee patted the fire out then noticed a minor burn on back of EE's calf.
EE was climbing on excavator after changing hydraulic hose to retrieve rags. While climbing down EE missed the last step with EE's foot and fell to the ground. EE landed on EE's right side, EE's hard hat came off and EE hit the back of EE's head on a rock. The wound required 4 staples.
Employee hit their head/hard hat on an I-beam in the plant when standing up. Due to the stiffness and pain not going away, EE saw a doctor who gave EE a prescription for a muscle relaxer. No light duty or other restrictions were given.
Employee was chipping concrete from piping when a piece of material entered their eye.
Miner was inspecting primary crusher when EE's left hand made contact with a jagged edge on a lift leg. Miner cut left palm and received stitches.
Employee was lifting piping when they felt pain in sternal region.
Employee was taping 3 coning bore holes, stepped on a loose rock with a change in elevation which resulted in a sprained ankle.
Ee was using an impact wrench to remove bolts. A nut on one of the bolts turned and cut the ee's left middle finger. Seen at Golden Health, stitches required to close the cut, ee released to full duty.
EE was tightening the plug on the gear box when the wrench slipped causing EE's left forearm to get caught on a piece of metal catwalk material.
Employee was inserting lock pin as the rotor was shifted by another employee. The pin inadvertently shifted and struck employee's left index finger.
Employee was working on a screw conveyor motor and the wind blew lime dust in between the frame and side shields of the safety glasses into employees left eye.
Employee was cutting coal with a continuous miner. During the process, a small piece of metal struck EE in the left lower lip. The metal was removed and stitches were required to close the wound.
EE was operating a chain hoist while looking up when dust fell from the hoist. Some of the falling dust got around EE's safety glasses and entered EE's right eye causing irritation.
While attempting to place a bearing onto a shaft using a hydraulic press the shaft slipped causing it to make contact with the guard. The guard then struck the employee.
Employee spliced C1 conveyor belt and was tracking the belt when the laces came around and hit the strap that was holding the rear skirt rubber. Employee took a pry bar and pried up on the strip without locking out the conveyor first. Pry bar became wedged causing EE's left index finger to be smashed between the belt and the bar resulting in two lacerations on EE's finger.
Employee states they were operating haul truck when their eye began to tear up. Employee folded a paper towel and touched it to their eyelid and saw a small bug on paper towel after patting the eye. The eyelid began to swell and they went to the clinic (10-25-21). Employee reported this to the supervisor today and asked to be seen at clinic. Employee felt it was work related.
EE was hooking up home made cable trailer to the Ford F-350 shop truck. EE placed left hand on trailer coupler and right hand on trailer frame pulling forward when the coupler slid down over the ball, EE left ring finger was pulled in between ball and coupler.
Employee was working on CAT Excavator tracks attempting to reinsert a roller pin. While striking the pin with a hammer a sliver a metal broke off and enter EE's right calf. Metal was removed by emergency clinic and 10 stitches were given.
Employee stated: was using a grinder R2 bin when a piece of metal struck EE in the eye.
Employee was drilling a stack of mild steel plates with battery powered portable drill using a pilot point tip type drill bit. The drill bit caught while drilling the hole and twisted causing employee to feel a pop in their bicep area of the right arm. THIS INCIDENT BECAME A RECORDABLE ON 12/6/21 WHEN EMPLOYEE STARTED PHYSICAL THERAPY.
Employee was proceeding down the stairs, at the primary, to clock out for the evening. Employee placed left foot/leg on the first step. When employee went to bear weight on the leg, employee felt a pop in knee. Employee was diagnosed with a torn meniscus and sent the physical therapy beginning 11-02-2021. Employee is working full duty.
Employee and coworker were rotating a magnetic separator belt. The coworker lost grip on the belt causing the injured employee to lose balance and grip resulting in a backwards same level fall into a handrail and a fractured rib.
Employee was cleaning primary feeder out for repair work when they ran a finger across a sharp metal burr cutting finger that required stitches.
Mill Operator was attempting to unplug a circuit with an air-lance when the fittings contacted structural steel and separated from the compressed air line. While the operator was attempting to shut off the air, the compressed air hose disconnected from the whip-check and struck the operator in the back of the head causing a laceration. Employee returned to work the following day.
Setting the feeder 2 brks inby #202 spad in #4 entry, blocking the cats to get it high enough to go over the tail piece. Was throwing a crib under the cat on the offside, when EE struck their left hand against the feeder and the end of the crib block.
Was putting metal in a forklift basket from the ground and when the weight of the metal caused the basket to drop down and spring back up hitting the employee in the left side stomach/rib area.
The Employee was attempting to change a spring on the De-Slime Screen when the slate bar the employee was using slipped and struck employee in the nose.
Employee was shoveling rust from the floor. EE moved a ladder to clean around it and a piece of cue decking fell causing a laceration to EE's right hand.
Employee was cleaning the windows on the outside of the cab. As they were closing the passenger door their right index and middle finger was caught between the door and the door jamb of the Haul Truck.
EE was fixing a wash pump for the wash plant. EE stepped over the hitch of the trailer, and hit their leg on the door of the service truck, which caused EE's leg to be cut. It was the right leg, roughly 3-4 inches above the knee.
Employee stated employee was moving 12470 high voltage plugs and foot got caught and employee fell backwards and felt a pain in right hip
Employee was putting a miner chain together. EE was striking chain with a hammer when chain flew up and hit EE on left side of face leaving an abrasion and a small piece of metal in EE's left cheek.
EE was adjusting carriage after team members had issues with busting bags. EE lowered carriage manual adjuster twice, the 3rd time EE did this EE felt a strain in EE's left lower back.
Electrician was adjusting power cable on elevator when the leather of the back of the electrician's left boot was pinched between toe guard and floor. This resulted in a laceration that required 10 stitches. Was given a work as tolerated release and missed no scheduled shifts.
When entering the employee parking lot on 10/07/21, employee hit a pot hole and twisted EE's back. EE reported the condition at the morning tailgate. On 10/12/21, employee requested time off work to seek chiropractic treatment, and time off 10/13/21 to see employee's PCP about pain back EE claims is from hitting the pot hole in EE's personal vehicle.
Employee was using a hammer to loosen the inner drill steel on the drilling equipment when the hammer slipped out of their hand and struck employee on the left upper forearm. After being fully released 3 times, On 11/5/2021 employee is scheduled for Physical therapy. At this time appointments are not made yet.
Employee was putting on a life vest to go onto a barge. As they were putting the life vest on they were bitten by a small insect. Employee was taken to clinic to be evaluated and prescribed steroids for swelling.
Operator was changing out Classifier blades, and in the process of doing so, operator cut finger while pulling a blade. Operator was required to wear gloves, and was not.
While installing new tracks on the CAT miner, the mechanic was using a chain come-a-long to pull the slack so that EE could install the pins. With tension on the come-a-long the hook slipped off and struck EE in the right thigh, cutting EE's pants and leg.
IE was cutting wire mesh from coal rib (Damaged Mesh). A piece of mesh sprung back contacting left eye area and safety glasses when they pulled the mesh free. IE was examined by medical and received prescription eye drops as treatment.
Operator was lifting a cover to close a ball valve, when the lid slipped out of their hand causing the lid to pinch index finger.
764 Power screen overheated. EE opened radiator cap to check fluid level. Under pressure, fluid sprayed out and got onto EE's arm and face.
Employee slipped exiting out of the cab of the backhoe, missed the grab handle and fell backwards, hitting head on the ground. Employee was taking to the ER, got 3 staples, and released to work.
Employee was operating a farm tractor when the engine began to overheat, the employee lowered the loader and shut the tractor down. While opening the hood, the employee stepped over the loader tilt linkage when employee's foot slipped causing left knee to strike the loader tilt linkage resulting in a laceration about 3 inches below the knee requiring stitches to close the wound.
EE was transporting portable crusher from Columbus to O'Neill Nebraska. EE was unhooking air lines from back of semi at site when EE lost EE's balance and fell about 3 feet to the ground. EE felt fine after getting up having full range of motion. On 2 hour trip back to Columbus EE started feeling sore.
Loader operator noticed an A/C vent fell out of housing. While replacing A/C vent while A/C was running, finger closest to thumb on (L) hand encountered fan cutting finger tip.
REPORTABLE ONLY DUE TO SUTURES - Employee was on East C Track on the locomotive taking an empty single end car back to put in E3, when one of the sander lids came off hitting employee in the mouth causing laceration to lip requiring sutures and chipping front tooth.

Employee was using a metal hammer to drive collar on a shaft. While striking the hammer a piece of metal splintered off the hammer and struck the employee in the neck.
THE CREW HAD COMPLETED A CORE TUBE RETREIVAL AND WAS PREPARING TO REMOVE THE STUFFING BOX AND OVERSHOT. THE DRILLER'S ASSISTANT BEGAN UNTHREADING THE STUFFING BOX WITH A PIPEWRENCH AT WHICH TIME THE STUFFING BOX, OVERSHOT AND TUBE FELL ON TO THE ASSISTANT'S RIGHT FOOT FROM A HEIGHT APPROXIMATED AT 4 FEET.
EE is in the process of training on the roof bolter. EE was trying to get a hole started to install a bolt. EE dislodged a piece of rock and it mashed the tip of EE's pinky finger causing a laceration and fracture.
Employee had hearing test. It was determined employee has a Standard Threshold Shift (STS) in the left ear.
Employee alleges was exposed to COVID. Employee did not report anything to the plant. Employee has been out since 10/22/21 due to COVID. The plant received a lawsuit from EE's lawyer on 01/23/22. Only thing plant is aware of is the employee carpooled with another employee. Mask and social distancing are required at work.

Employee took chest x ray on 8-3-21 and was diagnosed with occupational pneumoconiosis, we was notified of EE's diagnosis on 10/22/21.
The Employee Receive 10% Awarded for occupational Pneumoconiosis. Employee is currently still employed.
Notification was received on 10/21/21 that this retired employee has received a 10% award for Occupational Pneumoconiosis. This incident was caused by the employee's failure to wear appropriate respiratory protection during EE's mining career. Employee did not violate any rules, regulations, or policies and has proper training. Employee retired on 7/29/2015.
EE diagnosed with bilateral carpal tunnel syndrome on 10/21/2021. Treatment / SX is pending at this time.

Employee reported that EE was navigating a side hill when EE felt something burn in EE's left shoulder.
Employee reported to their supervisor on 10/15 that they had been experiencing pain in both elbows intermittently for the past five months. They reported the pain gets worse when they lift heavy objects, move their elbows and hands outward, away from their body and use a strong grip force. The employee believed a jackhammering job that occurred on 7/2 contributed to the injury.

We was notified on 10/14/2021 that employee was diagnosed on 6/15/21 with occupational pneumoconiosis by a b reader and chest x rays.
This report is being filed due to some hearing loss detected during the annual audiometric testing.
Employee was performing a routine clean up task and started experiencing pain in right elbow. After a few months the employee was still experiencing pain. At that time we decided to take the employee to an orthopedic center where employee was diagnosed with tendinitis on 1/19/2022.

The mine received notification on 10/07/21 that this employee has been awarded 0.18% PPD award for occupational hearing loss. Employee was enrolled in a Hearing Conservation Program and was properly trained in the use and the areas where hearing protection is required. The incident occurred because the employee failed to properly use hearing protection throughout mining career.
Received a CWP diagnosis for EE on 10/5/2021.
A miner was diagnosed with COVID on 10/1/21. On 10/4/21, a second miner reported symptoms and tested positive for COVID. The second miner switched equipment with the first miner halfway through the shift, on the first miner's last day at work prior to becoming symptomatic. Based on further investigation, the second miner's exposure likely happened in the workplace.
While riding the bus out of the mine, the employee went in to cardiac arrest. CPR was initiated and continued until EMS arrived, employee was transported to a hospital and was resuscitated.
Employee was operating the bulk loader station when they experienced chest pain and right arm and upper back pressure and pain. 911 was called. Employee was taken to Medical Center for evaluation and testing. It was determined that they had experienced a bad anxiety attack.
Employee had a seizure was not injured. We did call a ambulance and ambulance came and took EE to the ER in Abilene, Tx. Waiting to hear back on if it was due to prior health issues.
EE and co-worker had just finished up their work assignment and was cleaning work area up, co-worker stated co-worker took some tools back to tool room and came back to work site area and found EE laying on floor unresponsive.

CTI driver was stepping out of truck on scales. Parking brake was not set and truck started to move. Driver was pinned between the truck and scale railing.
Employee was assisting another technician with crack repair on the roof of a P&H 4100 Rope Shovel. EE was communicating with other tech and stepped backwards falling through the danger tape and into an open hole.
