A FALL OF ROOF APPROX 4 FT THICK BY 70 FT LONG BY 20 FT WIDE OCCURRED IN #8 ENTRY. FALL WAS FOUND AT 1:30 AM BY EMPLOYEE WHO WAS IN THE AREA WITH A CREW OF MEN TO CORRECT ANOTHER ROOF CONTR OL PROBLEM.

EE (BURNER OPERATOR) WAS PREPARING TO LITE OFF #3 CEMENT KILN. HE OPENED THE BY-PASS VALVE INSTEAD OF THE (BECK-VALVE) REGULATING VALVE AND GAS FILLED THE COOLER AND PART OF THE KILN. EXPLOSIO N OCCURRED AT COOLER BOWING SIDES OF COOLER.
NUISANCE GROUND TRIP ON PRIMARY BREAKER AT #1 HOIST. DOWN A 5:45 AM AND UP WORKING AT 7:15 AM VERBALLY REPORTED TO R. HAYES. EE WERE NOT IN ANY DANGER.
A VOID WAS DISCOVERED IN THE INTAKE ROADWAY ON THE BELT TUNNEL PROJECT. THE VOID WAS CAUSED BY AROOF FALL ON AN UNDERLYING MINE(WMEFREDE #8) WHICH RESULTED IN THE INTERBURDEN CAVING IN & THUS CREATING THE VOID. THE VOID MEASURED 18' X 23' &WAS LOCATED 83' INBY SPAD # 1139.
A ROOF FALL HAD OCCURRED IN THE X CUT BETWEEN THE #1 ENTRY AND #2 ENTRY, 7 C'S FROM THE INTAKE SHAFT ON BOTTOM. THE FALL MEASURED 30' LONG, 17'WIDE, AND 6' HIGH. DID NOT AFFECT VENTILATION, T RAVEL, AND NO ONE WAS INJURED.
SHEARER HAD CUT FROM H.GATE TO NO. 43 SHIELD AND HAD STARTED TO TURN TOWARD H.GATE AND CUT MORE BOTTOM FOR PAN LINE TO PUSH OVER WHEN TAIL GATE DRUM IGNITED CHY. APPROX. 3"" OF BOTTOM WAS BEING CUT. OPERATORS TRAMMED SHEARER ABOUT 3 SHIELDS TOWARD H.G. AND EXT. FLAME WITH FIRE EXTINGUISHER.
ROOF FALL 1 BREAK INBY SPAD 5025 ON 4 RIGHT OFF SQUIRREL MAINS. NO MEN OR EQUIPMENT INVOLVED.
ROOF FALL LOCATED AT X-CUT 21 IN THE #2 AND #3 INTERSECTION, 2-3 X-CUT. #2 ENTRY IS ALTERNATE ESCAPEWAY #3 IN BLET ENTRY 90' WIDE 21' LONG 6-15' HIGH. AREA BEING DANGERED OFF. A PLAN WILL BE WROTE UP AND FALL AREA REHABILITATED.
THE #2 HOIST EXPERIENCED A DEFECT THAT CAUSED THE HOIST SLOW DOWN SWITCH TO BECOME STUCK IN THE SLOW MODE. THE MINE WAS EVACUATED AND THE SLOW DOWN SWITCH WAS REPLACED. THE HOIST WAS TESTED FO R PROPER OPERATION AND THE MINE WAS RE-ENTERED AT 2:15 P.M.
CUTTER BEGAN IN XCUT 6 TO 7 WHICH FELL OUT WHILE BEING MINED. IT THEN RAN FROM #6 TO #2 AND #6 INTERSECTION FELL. 28' LONG X 18' WIDE X 8' HIGH.

A PIECE OF EQUIPMENT HIT A GATE CLOSE SWITH AND CAUSED THE CABLE TO GROUND WHICH TRIPPED THE CIRCUIT BREAKER ON THE HOIST CONTROL. NEW CABLE AND SWITCH WAS INSTALLED. THERE WAS NO ACCIDENT, IN JURY OR ILLNESS.

A ROOF FALL WAS DISCOVERED BY THE DAYSHIFT PRESHIFT EXAMINER WHILE DOING HIS PRESHIFT. THE ROOF FALL WAS IN THE 2ND INTERSECTION BACK FROM THE #10 SEAL IN THE MAIN EAST. THE ROOF FALL WAS 25' LONG, 18' WIDE & 32"" HIGH. THIS AREA WAS MINED BACK IN 1992 & BOLTED WITH 30"" FULLY GROUTED ROOF BOLTS.
A ROOF FALL WAS DISCOVERED BY THE DAY SHIFT. PRESHIFT EXAMINERS AS HE WAS MAKING HIS PRESHIFT CHECKS 1-26-2000. THE ROOF FALL WAS BETWEEN THE #9 & #10 SEAL IN THE MAIN EAST. THE FALL WAS 30' L ONG, 18' WIDE & 18"" TO 24"" HIGH.

ROOF FALL OCCURRED 40 FEET LONG, 15 FEET WIDE IN NO. 7 ENTRY, 80 FEET INBY SPAD 1085, APPROX 250 FEET OUTBY PILLAR LINE ON NORTH SECTION. THE AREA HAD BEEN BOLTED WITH 48"" RESIN BOLTS. BREAKER POSTS WERE SET AROUND THE FALL.
A PIECE OF ICE FELL FROM THE SHAFT WALL ADN DAMAGED THE UNIT SWITCH ON THE #9 BOTTOM, CAUSING THE MAN CAGE TO NOT OPERATE.

COAL TRUCK WAS WITHIN ONE BUCKET OF BEING LOADED IN THE PIT. A SECTION OF THE HIGHWALL FELL IN ON THE FRONT SECTION OF THE TRUCK. THE TRUCK WAS CONSIDERED A TOTAL LOSS. THERE WERE NO INJURIES AS THE RESULT OF THE EVENT.
AN UNINTENTIONAL FALL OF ROOF WAS DISCOVERED AT BREAK #22 ON NORTHEAST SUBMAINS. THE FALL WAS APPROX. 30' X 20' X 4-6'. THIS IS NOT LOCATED IN AN ACTIVE AREA OF THE MINE.
AT 11:15 PM THE MINER CUT THE 3 RIGHT BREAK INTO THE #4 HEADING AFF THE #8 X-CUT. METHANE HAD ACCUMULATED INT HE #4 HEADING. THE ROOF IN #4 WAS CRACKED AND APPARENTLY A METHANE BLEEDEY HAD REL EASED INTO THE FACE. THE MINER WAS CUTTING THE SANDSTONE ROOF. WHEN IT HOLED THROUGH INTO THE #4 HEADING WHEN THE METHING IGNITED. THE IGNITION TRAVELED APPROX. 60 FEET DOWN THE #4 HEADING. N
THE #6 AIRSHAFT ELEVATOR MALFUNCTIONED DUE TO A BROKEN RELAY. THE ELEVATOR STOPPED IN THE SHAFT SEVERAL TIMES WITH 16 PEOPLE ON IT. THE ELEVATOR WAS RUN ON INSPECTION SPEED TO CONVEY PERSONNEL UNDERGROUND AND WAS THEN TAKEN OUT OF SERVICE FOR REPAIR. ANOTHER ESCAPEWAY WAS TEMPORARILY USED.

THE ROOF IN THIS AREA WAS SUPPORTED WITH 4' FULLY GROUTED ROOF BOLTS AND 4 ROWS OF TIMBERS. ROOF FALL IS 30' LONG 20' WIDE 5' HIGH AND IS IN A 4-WAY INTERSECTION. THE ROOF FALL WILL BE CLEANED UP.

A ROOF FALL OCCURRED ON THE 1ST LEFT SECTION, #3 ENTRY AT THE FACE AND EXTENDING 30 FEET OUTBY. A LARGE CLAY VEIN ON THE LEFT RIB CONTRIBUTED TO THE FALL. SOME GLOVING EFFECT WAS DISCOVERED ON 4 ROOF BOLTS. THE USE OF LARGER BOLTS MAY ELIMINATE THIS OCCURANCE.
AT 11:10 P.M. ON 1-17-00 A FALL WAS DISCOVERED IN 5 ENTRY AT 3 CROSSCUT OF THE NORTHEAST MAINS. IT WAS APPROX. 1 CROSSCUT INBY SPAD 6 AND 3 CROSSCUTS FROM THE SURFACE. IT MEASURED 6'HX18'WX35' L AND LOCKED THE INTAKE ESCAPEWAY. ESCAPEWAY WASREROUTED THROUGH A 5' HIGH X 8' WIDE DOOR IN #4 STOPPING AND CONTINUED OUT #4 ENTRY TO SURFACE. FALL WAS REPORTED BY PHONE TO STATE AND FEDERAL
AN UNPLANNED ROOF FALL OCCURRED AT 1:00 AM ON 1-17-2000. THE ROOF FALL WAS FOUND ON THE 001-0 SECTION NEAR STATION SPAD #3401. THE FALL MEASURED 130' X 19' X 8'. NO EQUIPMENT WAS INVOLVED AND THERE WERE NO PERSONAL INJURIES. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERED OFF AND THE FALL WILL BE REHABILITATED.

A ROOF FALL HAS OCCURRED ON THE 001-0 SECTION IN THE #4 BREAK AT 5-A BELT. THE FALL MEASURED 19' WIDE, 60' LONG, AND 5' THICK. VENTILATION NOR PASSAGE OF PERSONS WAS AFFECTED AND NO INJURIES W ERE REPORTED. THE AREA WILL BE CLEANED UP AND RESUPPORTED.
WE GOT THE ALARM ON THE #3 KILN FIRING SYSTEM. THIS AUTOMATICALLY STOPS THE COAL BELT, SCREW, & ROTARY AIRLOCK THE FEEDS THE COAL MILL. AFTER ALARM CLEARED THE COAL FEED GROUP WAS RESTARTED. A LOUD NOISE WAS HEARD FROM THE COAL MILL AREA. (THE BELT & SCREW HAD NOT BEEN STARTED.) OCCASIONAL SPARKS COMING OUT OF THE COAL TRAP WERE OBSERVED. RUPTURE DISC ON TOP OF CYCLONE WAS DAMAGED.
AN UNPLANNED ROOF FALL WAS FOUND IN THE #2 ENTRY- 1300 OUTBY THE SECTION 1 1/2 OUTBY SURVEY STATION 5863. THE FALL MEASURED 19'L X 19'W X 6 1/2 - 7 1/2'H.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON 1-14-00 BY MINE FOREMAN. ALL ENTRANCES HAVE BEEN BREAKERED OFF TO THE AFFECTED AREA. THE FALL MEASURED 150 FT LONG X 18 FT WIDE X 7 FT HIGH. THE LO CATION WAS AT 1 CROSSCUT OUTBY SURVEY STATION #10589 - 700 FEET FROM SURVEY STATION.

THE MINER WAS CUT 6 LEFT, WENT TO FACE, 6 LEFT FELL IN. THE MINER RAMPED UP ROCK. DOUBLE BOOM PUT UP 5 ROWS OF PINS. ROOF BOLTER WAS BACKING OUT, ONE MAN WALKED TO SIDE CONTROLS, THE OTHER MAN STOOD BESIDE THE BOLTER. RIB ROLLED OUT 7 1/2' X 24"" X 22"" IN SIZE. PINNING INJURED AGAINST THE BOLTER.
DEMOLITION OF STEEL STRUCTURE USING CUT PULL AND DROP METHOD. A PIECE BECAME LOOSE AND ROLLED UNTIL IT HIT THE DECEASED.
EMPLOYEE WAS CAUGHT BY THE AUGER OF THE CONTINUOUS MINER RESULTING IN FATAL INJURIES.
ON 1/10/00 AT APP 1:30 PM I AND WORKER WAS SERVICING THE SCREENING PLANT WHEN FOOT FEEDER ON THE RUBBER TIRE LOADER HUNG. I THEN ATTEMPTED TO BEND OVER TO RELEASE THE FOOT FEEDER WHEN MY SHOUL DER BUMPED INTO THE BUCKET CONTROL LEVER CAUSING BUCKET TO TILT.
EE WAS WORKING IN AN UNAUTHORIZED AREA. HE PUT HIS ARM BETWEEN TWO ROLLERS, TO CLEAN ONE OF THEM OFF. THE ROLLER CAUGHT THE SLEEVE ON HIS JACKET TWISTING AND PULLING OFF HIS ARM AT THE ELBOW.
INJURED WAS CHECKING MOTOR ON PALLETIZER DUE TO FAILURE. PALLETIZER OPERATED AND THE PRESS ELEMENT OF THE PALLETIZER CAUGHT HIS RIGHT HAND CRUSHING THE ENDS OF 3 FINGERS, REMOVING LAST JOINT O F SMALL FINGER AND RING FINGER.
EE WAS STRAIGHTENING A DRAW BAR MANUALLY WHEN HE CLAIMS HE STRAINED HIS GROIN AREA.

EE WAS HELPING POSITION LOADER BUCKET ONTO FLAT BED TRUCK. A CHAIN WAS BEING USED TO POSITION THE BUCKET. THE INJURED EE PLACED HIS RIGHT HAND AROUND THE CHAIN. WHEN THE BUCKET WAS LIFTED THE RIGHT HAND WAS CAUGHT BETWEEN THE CHAIN AND THEBUCKET.

EE WAS LIFTING A METAL DISCONNECT STAND THAT WEIGHED APPROX. 100 LBS. AND FELT SOME DISCOMFORT IN THE GROIN AREA.
CLIMBING UP LADDER OF CINDER TRUCK. FEET SLIPPED AND HE CAUGHT HIMSELF BY HIS HANDS ON THE RUNG OF THE LADDER. HE HUNG BY HIS ARMS AND THEN LET HIMSELF DOWN TO THE GROUND.
ICY CONDITIONS. TRUCK DRIVER SLIPPED ON ICE WHILE ENTERING TRUCK. DISLOCATED RIGHT THUMB WHEN HE FELL.
EE WAS PULLING A WELDING CART ACROSS THE UNEVEN SURFACE OUTSIDE THE MAINTENANCE SHOP. HE TRIPPED AND PULLED THE CART DOWN ON TOP OF HIMSELF. HE RECEIVED SOME BRUISES AND STRAINED THE GROIN AND STOMACH. HE CONTINUED TO WORK UNTIL 2/7/00 WHEN HE WAS SENT TO THE HOSPITAL FOR MEDICAL ATTENTION. HE RETURNED TO HIS JOB WITH SOME LIGHT DUTY MODIFICATIONS.
WHILE MECHANIC WAS WELDING ON DRILL, THE DRILLER GLARED AT WELDING AND BURNED EYE.
EE AND COWORKER WERE WORKING TOGETHER AT THE TRACKS OF THE MINING UNIT. WHILE USING PROPANE TORCHES TO THAW FROZEN TRACKS OF MINING UNIT, A LARGE PIECE OF CLAY MATERIAL (~500-700#) FELL FROM A BOVE (~12') SCREEN AND HIT EE ON THE BACK. EE WAS KNOCKED TO THE GROUND. THIS INJURY REQUIRED XRAYS, PRESCRIPTION MEDICINE, AND MISSED DAYS.

THES E MOST RECENT ABSENSES WERE A RESULT OF THE INJURY SUFFERED IN 1989 (9/19/89) AND NOT CAUSED BY A NEW ACCIDENT OR AGGRAVATED BY HIS WORK ACTIVITY.

WALKING T=IN ENTRY AND TWISTED HIS KNEE WHEN HE STEPPED IN A HOLE. HE CONTINUED WORKING UNTIL 2/16/00. WAITING ON MEDICAL BEFORE TURNING REPORT IN.
EE CLIMBED UPON 769C HAUL TRUCK. HE WENT TOWARDS DOOR AND SLIPPED ON ICE ON THE CATWALK.

EE WAS BLOWING SOOT BACK OF #1 BOILER. WHEN HE WAS PULLING THE CHAIN, HE FELT A PAIN IN HIS BACK WHICH SHORTLY CARRIED TO HIS RIGHT ARM. PHYSICAL THERAPY STARTED ON 2/14/00.
EE WAS STARTING TO GET INTO HIS DRILL AND HE PUT HIS FOOT ON THE 1ST STEP AND WHEN HE PULLED HIS WEIGHT UP HIS FOOT SLIPPED OFF THE STEP CAUSING HIS LEG TO TWIST ON IMPACT WITH THE GROUND. THE INJURY WORSENED AS TIME WENT BY CAUSING EE TO SEEK MEDICAL TREATMENT. EE TOLD THIS TO THE BOSS.
EMPLOYEE WAS WALKING TO HIS CAR TO GO TO A MEETING AT NAZ 3 AND SLIPPED ON SOME SNOW/ICE AND FELL. IN AN EFFORT TO KEEP HIS BALANCE EMPLOYEE MADE A QUICK MOVEMENT WITH HIS HEAD.
EMPLOYEE WAS HELPING MOVE A TRACK SWITCH WHEN HIS FEET SLIPPED AT THE SAME TIME SWITCH SLID HITTING LEFT LEG ABOVE ANKLE.
EE WAS LOADING 6' POINT ANCHOR BOLTS INTO A SCOOP BENDING AND TWISTING AND SUFFERED A LOWER BACK MUSCLE STRAIN. CURRENTLY RECEIVEING PHYSICAL THERAPY. HE IS CURRENTLY UNABLE TO WORK.
EE WAS ASSIGNED TO RECHARGE DISCHARGE END OF THE FINISH MILL #2. A BARREL OF BALLS WEIGHING 1 TON WAS LIFTED TO THE TOP OF MILL WITH CRANE AND EE WAS DUMPING BALLS INTO THE HOPPER PLACED AT MI LL. THE BARREL WAS APPROX. 3/4 EMPTY, ABOUT 500 LBS REMAINING, WHEN IT JUMPED OUT OF THE HOPPER CAUSING EE TO JAM RIGHT ARM BETWEEN HOPPER AND BARREL.
THERE WAS A RIB ROLL THAT STRUCK THE EE. HE SUSTAINED ONLY A CONTUSION, BUT BECAUSE OF A PRE-EXISTING CONDITION, THE DR. KEPT HIM OVERNIGHT FOR TESTS AND OBSERVATION. HE WAS SENT HOME THE NEXT DAY. TWO WEEKS LATER THE DR. FOUND FLUID THAT HAD TO BE DRAINED FROM THE CONTUSION. BECAUSE OF THE DR.'S VISITS. THIS BECAME A LOST TIME ACCIDENT.
EMPLOYEE STATED HE WAS UNLOADING TIMBERS OFF OF SCOOP BUCKET AT HEADING TO BE SET ON PILLAR LINE
EE SLIPPED OFF THE LAST STEP OF #2136 TRUCK. HE FELT NA DFELT A SHARP PAIN LEFT SIDE OF BACK.
EE WAS BOLTING ROOF IN #2 HEADING, WHEN A PIECE OF DRAW ROCK FELL ON HIS LEFT ANKLE.
EE WAS BEGINNING TO HOIST AN I-BEAM USING A CHAIN HOIST WHEN THE LOAD SHIFTED AND THE I-BEAM ROLLED COMPLETELY OVER FALLING ON THE TOP OF HIS FOOT.
EE WAS TAKING A LOAD OF ABC TO THE THE PIT FLOORWITH A CAT 980C LOADER WHEN THE BRAKES FAILED. EE JUMPED OUT AND LANDED ON HIS LEFT LEG BREAKING THE BONE(FEMUR). THE LOADER ROLLED ABOUT 40 F EET AND STOPPED AT A BERM. THE BUCKET WAS NOT LOWERED AND THE PARK/EMERGENCY BRAKE WAS NOT ACTIVATED.
EMPLOYEE OPENED INSPECTION CAP ON GATE TO RETRIEVE POSSIBLE CHUNKS OF CEMENT PLUGGING GATE. CEMENT STARTED TO FLOW OUT FROM THE INSPECTION GATE. WHILE TRYING TO REPLACE CAP, CEMENT BLEW INTO H ER EYES.
EMPLOYEE WAS WALKING TO THE DRIVER'S SHED FROM HIS PERSONAL VEHICLE PRIOR TO THE START OF HIS SHIFT WHEN HE SLIPPED ON SOM ICE AND TWISTED LOWER BACK AREA.

WHILE PULLING SUPPLY CART ON ELEVATOR THE 58 HORSEPOWER PUMP SKIDDED AND CAUGHT FINGER AGAINST HANDLE.
MAN WAS WALKING DOWN CATWALK. IT HAD SNOW & ICE ON IT. HE SLIPPED AND FELL, HITTING HIS HEAD ON HANDRAIL.
INJURED STRAINED HIS NECK AND SHOULDER WHILE ATTEMPTING TO LIFT A PIECE OF PLYWOOD.

EE WAS REPLACING THE DUST COVER ON THE HEAD PULLEY MISSED A STEP AND FELL TO THE GROUND BREAKING HIS ANKLE.
EE AND COWORKER WERE LIFTING THE LONGWALL RACK BAR (3' LONG +120#) WHEN COWORKER'S GRIP SLIPPED CATCHING EE'S LEFT MIDDLE FINGER BETWEEN THE BAR AND THE FACE CHAIN.
SEARCHING FOR KEYS TO BOBCAT, WHEN SAFETY BAR FELL AND HIT BACK OF HEAD. WORK ORDER FOR REPAIR OF SAFETY BAR WRITTEN 12/99. CLARKLIFT WAS WAITING FOR PARTS. EE WAS UNAWARE OF PROBLEM. POOR COM MUNICATIONS CONTRIBUTED TO THE INJURY.
EMPLOYEE WAS GETTING OUT OF LUBE TRUCK AS HIS FEET TOUCHED SNOW COVERED ICE, BOTH FEET SLIPPED OUT FROM UNDER HIM. HE CONTINUED TO WORK UNTIL 2-21-2000.
EE WAS OPERATING THE SHEAR, SHEN A ROCL FELL FROM BETWEEN THE SHIELDS STRIKING THE EE'S RIGHT HAND THAT WAS LAYING ON TOP OF THE HANDRAIL. THIS RESULTED INTO A LACERATION, WHICH LATER BECAME I NFECTED WITH THE FIRST LOST DAY WAS 2/1/2000.
EE WAS WORKING AT AN ELEVATION, THREADING CONDUIT. HE WAS PULLING DOWN ON THREADER HANDLE. THE HANDLE CAUSED HIM TO FALL BACKWARD AND HIT THE LEG ON SCAFFOLDING.
HE WAS WORKING ON A ROCK TRUCK AND INJURED HIS LEFT FOOT DUE TO SNOW AND ICE.
HE WAS BOLTING TOP AND HURT HIS LOWER BACK
EE STATES HE FELT PAIN IN HIS SHOULDER WHEN HE WAS DRIVING THE LOADER. HE TOUGHT IT WOULD GO AWAY. WHEN HE GOT UP THE NEXT MORNING IT WAS PAINFUL ENOUGH TO GO TO THE HOPSITAL.
EMPLOYEE WAS ATTEMPTING TO HELP STRAIGHTEN UP THE BELT DRIVE WITH A PRY BAR. AFTER THE BELT DRIVE HAD PLUGGED UP AND MOVED IT OUT OF ALIGNMENTTHE BAR SLIPPED STRIKING HIM IN THE FRONT OF THE JAW. CAUSE OF INJURY WAS AWARENESS.
THE EE WAS WALKING ON THE SURFACE NEAR THE BELT ENTRY TO THE MINE. HE LOST HIS FOOTING ON FRESH FALLEN SNOW AND FELL TO THE GROUND. HE TWISTED HIS LEFT KNEE AS HE FELL.
ON 1/28/00 THE EE WAS WALKING AROUND A ROOF BOLTER AND FELL WHEN HE STEPPED ON SOME SLICK BOTTOM. THE EE TWISTED HIS LEFT KNEE WHEN HE FELL. THE EE DID NOT MISS WORK BUT HAD KNEE SURGERY ON MA RCH 14, 2000.
EMPLOYEE WAS INSTALLING THE LAST ROW OF 6' BOLTS AT THE TIME OF THIS INCIDENT. IT WAS LOW ENOUGHAT THE FACE THAT HE HAD TO BEND THE BOLT TO INSTALL IT. HE PUSHED THE BOLT JUST FAR ENOUGH INTO THE ROOF TO STRAIGHTEN IT A LITTLE TO GET IN THEDRILL. IT FELL AND STRUCK HIM ON THE RIGHT FOREARM. THIS CAUSED A LACERATION IN HIS FOREARM ABOUT 2 1/2"" LONG.
EMPLOYEE WAS LOADING A PATTERN ON THE SECOND SOUTH EAST AREA OF THE PIT. HE STEPPED ON A ROCK AND TWISTED HIS RIGHT KNEE. HE INITIALLY SOUGHT MEDICAL CARE ON 2-7-00 AND RECEIVED A FULL DUTY RE LEASE WITHOUT RESTRICTIONS. IT WAS DISCOVERED HEHAD A LIGAMENT TEAR THROUGH FURTHER DIAGNISTIC TESTING.
FELL WHILE WALKING AT THE BOTTOM AREA BRUISING AND STRAINING KNEES.
WHILE SHOVELING #1 BELT, EMPLOYEE TWISTED TO THROW COAL ON BELT AND PULLED MUSCLE IN BACK. EMPLOYEE WORKED UNTIL 6/1/00 WHEN HE WENT OFF & GOT OPERATION. REPORT WAS DELAYED UNTIL COMP. WAS APPROVED.
THE EMPLOYEE WAS WORKING AROUND TAIL PIECE AND FELL INTO THE BELT CAUSING A BRUISING OF HIS LEG.

ON 1-27-00 EE INJURED HER NECKK WHEN HE R-190 END DUMP TRUCK JERKED WHILE DUMPING A LOAD OF DIRT. THIS WAS REPORTED ON 1-28-00 AND ON 1-29-00 SHE REQUESTED MEDICAL ATTENTION. SHE WAS TAKEN TO THE LOCAL E.R. AND DIAGNOSED WITH A PULLED MUSCLE. SHE WAS NOT SCHEDULED TO WORK ON JAN 30,31 OR FEB 1 ST. SHE VISITED HER FAMILY PHYS 1-31-00 AND DIAGNOSED WITH A CERVICAL STRAIN.
MECHANIC TRIPPED AND FELL HURTING HIS KNEE.

EMPLOYEE ALLEGES HE TWISTED HIS RIGHT KNEE WHILE WALKING #3 BELT. EMPLOYEE STATED HE STEPPED ON A ROCK. NOTE: DID NOT SEEK MEDICAL ATTENTION UNTIL 2/4/00 THEN STARTED MISSING WORK AFTER THAT D ATE.
THE LOADER BRAKES HAD BEEN FIXED. A LEAKAGE OF BRAKE FLUID CONTINUED AND THE MECHANIC WAS CALLED TO FIX IT. THE OPERATORS WERE DOING THE INSPECTIONS AT THE BEGINNING OF EACH SHIFT AND ADDING T HE NECESSARY BRAKE FLUID. THIS OPERATOR DID NOT MAKE THE BEGINNING INSPECTION AND THE EQUIPMENT RAN OUT OF BRAKE FLUID CAUSING THE BRAKE FAILURE.
INJURED EE WAS ATTEMPTING TO CONNECT AN AMP METER COUNTED ON AN ELECTRICAL CABINET. ALTHOUGH STILL UNDER INVESTIGATION, IT APPEARS THAT ELECTRICITY ARCED TOWARDS HIS RIGHT ARM. THE ELECTRICITY , (4160 VOLTS) SOUGHT GROUND TO THE CABINET, MISSING HIS ARM. HE RECEOVE AN ELECTRICAL SHOCK THROUGH CONTACT WITH THE CABINET.
EE WAS OPERATING A ROOF BOLTER IN THE #5 WORKING FACE. EE HAD BROKEN A BIT OFF AND WAS ATTEMPTING TO GET THE DRILL STEEL OUT WHEN HE CAUGHT HIS LEFT INDEX FINGER BETWEEN THE DRILL POT AND THE MINE ROOF.
WHILE RUNNING THE DOZER IN THE PIT, RAN OVER A LARGE ROCK. WHEN THE DOZER CAME DOWN WITH A JAR, EE EXPERIENCED PAIN IN HIS LOWER BACK.
EE WAS CLIMBING UP STEPS OF END LOADER AND STARTED TO OPEN DOOR OF END LOADER WHEN HE SLIPPED AND FELL.
AFTER WORK AND THE EE HAD CLOCKED OUT FOR THE DAY THE EE HAD STARTED WORKING ON A PERSONAL PROJECT IN THE SHOP. THE EE STEPPED ON A CRACK IN THE CONCRETE FLOOR HIS KNEE BUCKLED AND HE FELL HIT TING HIS KNEE AND FRACTURE HIS KNEE-CAP.
TORE OR PULLED A MUSCLE IN LOWER LET LEG INSTALLING A TAIL ROLLER BEARING.
EE WAS WALKING TO TOP OF MIXING BINS, SLIPPED ON ROCKS ON CATWALK AND FELL ON HIS BAKC, INJURING HIS BACK. * THIS FORM WAS COMPLETED LATE DUE TO EMPLOYEE NOT REPORTING LOST TIME UNTIL 2/4/00.
THE DRIVER OF THE LOADER IGNORED HIS SUPERVISOR'S WORK ORDERS AND EXCAVATED A HIGH EMBANKMENT. THIS RESULTED IN A ""CAVE-IN"" OF MATERIAL. THE LOOSENED MATERIAL WAS UNSTABLE AND FELL ON THE LOAD ER BUCKET. THE LOADER WAS JARRED WHEN THE MATERIAL FELL ON THE BUCKET.

EE CLIMBED FROM ONE FORKLIFT TO ANOTHER, THE OTHER FORKLIFT WAS WET AND SNOWY. HE DID NOT SIT DOWN, PUT THE FORKLIFT IN REVERSE. WHILE MOVING, HIS FOOT SLIPPED FROM THE FORKLIFT AND WAS CAUGHT BETWEEN THE TWO FORKLIFTS. HE PANICKED AND PULLED HIS LEG OUT FROM BETWEEN THE TWO.
EE WAS PULLING ON MINER CABLE TRYING TO HANG TO CABLE TO MINE ROOF WHEN HE STRAINED RIGHT ARM.
EE WAS DRIVING A TRUCK AND BACKING UP, WHEN ANOTHER VEHICLE HIT HIM IN BACK TAILGATE.
F-BLORS AS THE EMPLOYEE WAS BENDING OVER TP PICK UP A VENT TUBE HE STEPPED OVER THE MINER CABLE AND WATER HOSE. AT THE SAME TIME TWO MEN LIFTED THE CABLE AND HOSE CATCHING HIS FOOT AND CAUSING HIM TO FALL. HE STUCK HIS ARMS OUT TO BREAK THEFALL. RESULTING INJURY WAS A FRACTURED SOCKET IN THE LEFT SHOULDER JOINT.
EE WHO HAS CHRONIC BACK PROBLEMS WHICH HE STATED ARE NOT RELATED TO THIS OCCURANCE BEGAN HURTING HIM AFTER HE HAD HANDLED MINER CABLE.
PIT LABORER WAS MANUALLY MOVING SHOVEL ELECTRICAL POWER CABLE. A SECTION WAS FROZEN IN SNOW/ICE. HE ATTEMPTED TO PULL IT LOOSE AND FELT SOMETHING GIVE IN HIS LEFT SHOULDER. THE PAIN INCREASED OVERNIGHT AND HE REPORTED THE INCIDENT. HE WAS SUBSEQUENTLY DIAGNOSED WITH A CHEST STRAIN. THIS INCIDENT OCCURRED AT A GROUP II SWITCHHOUSE IN THE MINE PIT.
MAIN VALVE OF MIX MASTER STUCK OPEN. EE OPENED NORTH DOOR AND CLEANED IT OUT. TRIED TO START MIXER BUT WOULD NOT START. WHEN HE TRIED TO OPEN DOOR IT BLEW DUST AND STEAM ON HIS HANDS AND FACE.
EE WAS UNLOADING A SMALL POWER POLE, AND HIS HAND SLIPPED OFF. THIS CAUSED A TEETER TOTTER EFFECT WITH THE POLE. THE POLE WAS SUPPORTED AT THE CENTER POINT WITH A SET OF POLE TONGS. AS THE TOP OF THE POLE CAME IN CONTACT WITH THE GROUND IT FORCED THE POLE TONGS TO RELEASE. THE POLE HIT THE GROUND AND ON THE BOUNCE IT HIT HIM IN THE LEG.

THE IMPACT THREW HIM INTO THECANOPY POST BREAKING HIS CHEEK BONE AND GIVING HIM A SLIGHT CONCUSSION.

EMPLOYEE HAD SURGERY ON 6/13/00 TO REPAIR A TORN ROTATOR CUFFAND BEGAN MISSING WORK.

EMPLOYEE HAD JUST INSTALLED THE BELT CLAMP TO SPLICE THE BELT, AND WHEN THEY PUSHED THE CLAMP BACK ONE END CAME LOOSE AND THE CLAMP CAUGHT HIS LEFT THUMB, THIS RESULTED IN A CUT TO HIS LEFT THUMB.

ROOF BOLT LOST CONTACT W/ RETAINING STRAP & FLEW UPWARD, HITTING HIM ON CHEEK CAUSING ABRASION. WENT OFF 1/24 WITH INFECTION.

EE WAS ASSISTING A WELDER ON THE #2 COMPARTMENT FAN PROJECT. HE WAS BENT OVER THE FAN TO HELP HODL A PART IN PLACE AND APPARENTLY TWISTED WRONG,. HE FELT PAIN IN HIS BACK A SHORT TIME LATER AN D TAKEN TO DR. HE WAS DIAGNOSED WITH A MUSCLE SPASM AND GIVEN PRESCRIPTION MEDICATION FOR PAIN AND A MUSCLE RELAXANT.

AT THAT TIME THE REPORTED INJURY WAS TREATED AS A FIRST- AID CASE. ON 01-19-00 THE SAFETY SUPERVISOR WAS INFORMED THAT EMPLOYEE HAD DEVELOPED COMPLICATIONS CONCERNING THE INJURY. HE WAS TRANSPORTED TO SEEK PROFESSIONAL CARE, WHERE HE WAS DIAGNOSED
EMPLOYEE WAS OPERATING ROOF BOLTER, HE WALKED AROUND THE ROOF BOLTER AND TRIPPED. HE FELL ON A ROOF BOLTER PLATE AND SUFFERED A MINER LACERATIONON HIS RIGHT THIGH.
MINER CABLE WAS LAYING ON HILL. EE STEPPED ON CABLE CAUSING HIM TO SLIP AND FALL.
WHILE EXAMINING THE MAIN EAST #1 BELT, EE STRUCK HIS HEAD ON A ROOF BOLT. HE HURT HIS BACK & JAMMED HIS NECK. MAIN EAST BELT 16 TO 17 XCUT).

EE WAS STEPPING OVER CM CABLE AND WATER HOSE AND CAUGHT HIS FOOT ON THE WATER HOSE AND TRIPPED. EE BECAME A LOST TIME INJURY JANUARY 23.

EMPLOYEE BACKED HIS DOZER INTO A ROCK AND CAUSED A WHIPPING EFFECT WHICH STRAINED HIS NECK AND RIGHT ARM.
EMPLOYEE WAS CLIMBING OFF OF THE TRAK HOE. HIS FOOT SLIPPED ON A OILY SPOT AND HE TWISTED HIS RIGHT KNEE.
EE WAS RUNNING A LOADER WHILE LOOSENING DIRT IN QUARRY. WHILE DOING SO, LOADER HOOKED STONE JERKING LOADER AND EE WITH SAFETY SEAT BELT ON SIDE WAYS CAUSING EE TO HIT THE ARM REST OF HIS SEAT LEFT SIDE.

FUELING UP FUEL TANK, PULLING ON FUEL HOSE AND WRAPPING THE HOSE UP ON THE DECK, PULLED UPPER RIGHT ARM. (NO MEDICAL ATTENTION WAS REQIURED TIL 1-28-2000).
EMPLOYEE WAS TRAVELING #4 ENTRY TO THE SURFACE ON 3-WHEELER. HE WAS LOOKING AT A POWER CURTAIN HE WAS PASSING BY AND CAUGHT HIS HEAD ON THE ROOF HURTING HIS NECK.
EE WAS POSITIONED IN NO. 4 ENTRY WHEN SHUTTLE CAR OPERATOR MISJUDGE LOCATION OF XCUT, TRAVELING TOO FAR BEFORE TURNING INTO XCUT. THE SHUTTLE CAR STRUCK INJURED, CAUSING TRAUMA TO HIS ABDOMINA L AREA. (HE WAS NOT CAUGHT BETWEEN OBJECTS OR ROLLED).

THE OPERATOR WAS JARRED IN SEAT WHEN LOADER BUCKET HIT LARGE BOULDERS AND WALL.
EE WAS CHANGING OFF ROAD DUMP TRUCK TIRE WHEN TIRE FELL ON EMPLOYEE'S LEG.
THE EE WAS CLEANING SPILLAGE FROM THE WEST PITCHING BELT IN THE PRIMARY CRUSHER. AS HE PULLED MATERIAL AWAY FROM THE BELT HE BENT OVER IN AN AWKWARD POSITION. AS HE PULLED HE FELT IN HIS RIGHT SHOULDER. THE PAIN PERSISTED FOR SEVERAL DAYS AND AN EXAM WAS NECESSARY AND FOLLOW-UP PHYSICAL THERAPY WILL BE NEEDED.
11C SECTION HE HAD ROCK DUST HOSE ON HIS SHOULDER AND WAS DRAGGING HOSES INTO THE SECTION. A BREEZE BAND CAUGHT ON TRACK TIE AND STOPPED HIS ADVANCE. IT PULLED BACK HIS RIGHT WRIST CAUSING INJ URY. LOST WORK DAYS BEGAN 1-17-00.

EE WAS TRAMMING ROOF BOLTER. AS HE WENT OVER A ROLL, THE REAR TRAM COMPARTMENT SUDDENLY RAISED CAUSING HIS HEAD TO STRIKE THE ROOF. THIS RESULTED IN A SPRAIN TO HIS NECK. DID NOT GO TO DR. UNT IL 1-20-00. DID NOT MISS WORK UNTIL 1-21-00.
EMPLOYEE WAS USING CUTTING TORCH TO REMOVE BEARINGS FROM A BULLDOZER BLADE ASSEMBLY. WHILE KNEELING DOWN FOR A EXTENDED PERIOD OF TIME ON HIS KNEES, WHEN GETTING UP TO A STANDING POSITION HIS RIGHT KNEE POPPED.
WHILE HELPING THE C/M OPERATOR, EE NOTCIED A PIECE OF LOOSE ROOF NEAR THE OPERATOR. SHE WENT AROUND THE C/M TO GET THE SLATE BAR. THERE WAS LOOSE ROCK ON THE BOTTOM. AS SHE STEPPED OVER THE RO CK TO GET THE BAR SHE SLIPPED AND TWISTED HER RIGHT ANKLE.
EMPLOYEE HAD FINISHED REPAIRING THE KENWORTH STOCK TRUCK AND WAS STEPPING DOWN FROM THE CAB WHENHE MISJUDGED HIS DISTANCE AND TWISTED HIS LEFT KNEE UPON LANDING ON THE GROUND.
EMPLOYEE WAS WORKING ON A DRILL STEEL. HE WAS USING A TORCH ON THE OUTRIGGER OF HIS SERVICE TRUCK HE KNOCKED OFF THE RING AND THEN BACKED INTO THE LIT TORCH. HE WAS TRANSPORTED TO THE GREEN CO UNTY GENERAL HOSPITAL TO SEEK PROFESSIONAL CARE,WHERE HE WAS DIAGNOSED AND TREATED FOR SECOND DEGREE BURNS ON THE RIGHT SIDE OF HIS BACK.
ON 01-14-00 EMPLOYEE REPORTED THAT HE FELL AT A FLOOR DRAIN AND STATED THAT HE WAS NOT INJURED. HE CONTINUED TO WORK REGULAR UNTIL 02-18-00 WHEN HE INFORMED MANAGEMENT THAT HE WAS EXPERIENCING KNEE AND BACK PAIN FROM THE 01-14-00 INCIDENT. WENT TO DOCTOR 03-02-00 DID NOT TAKE OFF WEEK AS DOCTOR SUGGESTED. THIS EMPLOYEE HAD PREVIOUS NON WORK RELATED INJURY AND COMPANY WAITING ON POS
EE STATED HE HAD FUELED UP THE #2 REFUSE TRUCK IN PREPERATION TO HAUL REFUSE. HE STARTED UP THE ENTRANCE LADDER TO THE OPERATORS COMPARTMENT. WHILE ATTEMPTING TO CLIMB THE LADDER HIS WRIST GAV E OUT CAUSING HIM TO FALL BACKWARDS ABOUT FOUR FEET TO THE GROUND.
EE WAS HOOKING A WELDING CART TO A 3 WHEELER WHEN HE PULLED OR STRAIN A MUSCLE IN HIS BACK.
EE SLIPPED & FELL WHILE CARRYING A BAG OF ROCK DUST. EE SAID WHEN HE FELL HE LANDED ON HIS SELF CONTAINED SELF-RESCUER THAT IS ON HIS BELT.
EE ALLEGES HE WAS CLEANING OFF SAAND FROM SANDBLASTING WHEN HE PUT WEIGHT ON BLOCKS THEY FELL OVER AND HE FELL TO THE GROUND.

WHILE JACKING HIS JEEP ON TRACK THE JEEP SLID OFF THE JACK WHILE HE HAD HOLD OF THE JACK BAR. THE JACK KICKED OUT SUDDENLY. DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES AND REGULATIONS. MINING SYSTEMS NOT INVOLVED. JOB SKILLS, MINER PROFICIENCY, TRAINING OR PROTECETIVE ITEMS NOT A FACTOR. ATTITUDE COULD BE A FACTOR.
EE WAS PULLING CABLE SLACK FROM OFF THE CABLE REEL OF A FLETCHER ROOF BOLTER SO HE COULD HANG THE CABLE FROM THE ROOF. THE CABLE WAS HARD TO PULL OFF AND AFTER HE PULLED IT OFF HE SAID HIS RIG HT ARM AND SHOULDER BEGAN TO ACHE.
EE WAS CHANGING BRUSHES IN A 2550 DRAGLINE DRAG MOTOR. WHILE MOVING FROM ONE SIDE OF THE MOTOR TO THE OTHER, HE FELT PAIN IN HIS GROIN AREA. EE DID NOT SLIP OR FALL. THIS IS SOME AWKWARD BENDI NG AND MOVING WHEN WORKING AROUND THE MOTOR AREA. SURGERY SCHEDULED FOR 1-26-00.
EE WAS TARPING HIS TRUCK(AUTOMATIC TARPER) AND THE TARP BECAME ENTANGLED IN THE TARP FRAME. HE REACHED UP TO UNTANGLE THE TARP AND FELT A BURNING SENSATION IN HIS LOWER BACK (14TH). THE PAIN B ECAME PROGRESSIVELY WORSE UNTIL HE BEGAN LOSING TIME (24TH).
OPERATOR NORMALLY JUMPS FROM THE TRACK TO THE GROUND, BUT FORGOT THAT THE TRACKS WERE RAISED, ADDING ANOTHER SIX INCHES TO THE DISTANCE. RIGHT UPPER LEG COULD HAVE HIT THE TRACKS.
EMPLOYEE WAS CHECKING A COMPRESSOR, WHEN HE WAS CLOSING A VALVE, HE HURT HIS WAIST AND THIS PRODUCED SHARP PAIN IN THIS AREA. HE CONTINUED TO WORK BUT THE PAIN INCREASED.
EE WAS WALKING AT THE BOTTOM OF SERVICE SHAFT, WHEN HE SLIPPED AND TWISTED HIS LEFT ANKLE.
EE WAS HOLDING ONE END OF SEALANT MACHINE UP WHIOE ANOTHER EE PICKED IT UP WITH SCOOP AND LET THE SCOOP BUCKET DOWN ON EE'S FOOT.
BASED ON OUR INVESTIGATION THE EMPLOYEE WAS TRAVELING THE #2 CONVEYOR. THE NEXT TIME THE EMPLOYEE WAS PHYSICALLY SITED, HE WAS ON THE #2 CONVEYOR. THE EMPLOYEE ENTERED AN AREA TOO LOW FOR HIM TO PASS AND WAS CAUGHT IN BETWEEN THE MINE ROOF AND THE #2 CONVEYOR. ESTIMATED INJURIES TO APPROXIMATELY 50% OF THE BODY.
WORKER FELL OFF FRONT END LOADER AND TWISTED KNEE, HAD KNEE REPAIRED BY SURGERY ON 9-5-00. NEVER LOST ANY TIME UP TO THIS DATE.

THE EMPLOYEE WAS DESCENDING A SET OF 6 STEPS WHICH WERE WET PAINTED CONCRETE WHEN HE SLIPPED AND FELL BACKWARDS ONTO THE EDGE OF A STEP.
INJURED PERSON WS WALKING AROUND TAIL OF MINER. WHEN HE SLIPPED IN A HOLE TWISTING RIGHT KNEE. HE SUSTAINED A TORN LIGAMENT WHICH REQUIRES TIMEOFF WORK AND SURGICAL REPAIR.
