Mining Incidents in September 2006

Cutting coal on the longwall face at Nor 76 shield when an ignition occurred at the top of the headgate drum.

An ignition of methane occurred in the fresh water pump house causing damage to the structure. Inadvertently, methane was liberated from the well, accumulated in the structure and when the pump turned on the ignition occurred.

A roof fall occurred in the 15 North Panel #10 entry in the intersection of cross-cut #23 and extending through the pillar line toward #9 entry measuring 50ft. x 20ft. x 6ft.- 9ft. The area was supported with 5ft. fully grouted bolts and 9ft. mega bolts. An undetected slip was determined to be the cause of the fall. The area was mined in August 2006.

Roof fall size-length-40'; width-16'; height-6'; 48""resin;strata-shale; no coal rider seam present; all entrances are not breakered off due to excessive height, danger signs are in place. This area has been 2nd mined by pulling the bottom coal.

While operating the hoe, a newly installed hose blew an end & sprayed hot oil over the engine & turbos igniting a fire that quickly got out of control. Fire extinguishers had no effect. 1st fire department called deployed to the wrong place & a 2nd fire dept. had to be called. The fire depts did not fight fire, but made sure it stayed contained. Burned out @ 2:00pm no injuries

Roof fall size-length 140';width-16';height-8'.48"" resin; strata-shale; no coal rider seam present; entrances are not breakered off due to excessive height, danger signs are in place; area will not be rehabilitated. This area has been 2nd mined by pulling the bottom coal.

Rock fall inby spad 581, approx 100'.

An unintentional roof fall occurred at the #14 brk, 3 West #2 belt in the #3 entry. Size of fall was estimated to be (5'H x 18'-20'W x 70'L). Fall will not be cleaned up. No injuries occurred, no equipment caught, ventilation was not interrupted. Area had been bolted with 6' torque tension bolts.

Broken Brake Caliper on the 7-10 Hoist. No injuries.

Roof fall between spad #133 and #134 in #1 entry on 1 north mains. Fall measured 65 ft. long, 19 ft. wide by 8 ft. thick, approximately 9,200 ft. outby the section. Rotten top and water present was determined as the cause.

Fall happened in #3 entry at spad #19225-50 break.Fall was 40'l x 20'w x 4-6' thick. 6' torque tension bolts were in use. No injury or travel was blocked. Reported verbally (phon) to local MSHA office in Mt. Hope on 9/28/06.

An unintentional roof fall was discovered in the old 6 Miner #1 Belt entry. Height - 5'; Width - 18'; Length - 50'Support: 5' fully grouted resin & 10' cable bolts.

Brake gear box failure. No injuries, back-up systems went into effect.

A roof fall was discovered on the 4 East area that was approximately 100' long by 18' wide by 8' high. The fall will not be cleaned up.

ROOF FALL FOUND X-CUT 40 #2 UNIT NORTH SUBMAINS BETWEEN #6 & #7 ENTRIES 20' X 70' X 42""

Unintentional roof fall 100'(L) x 19'(W) x 4'(T) on 1 West Mains.

An unintentional roof fall occurred in the 7I section #3 entry at 62 + 20. The fall measured approx 20' L x 16' W x 9' high. No injuries nor equipment damaged.

Outby #4 unit. Belt entry (x-cut 9). Length 30', width 30', 8'-9' high.

Fall occurred appr. 800' from the portal at the 1 st overcase underground on the rock house track way. Area was dangered off and cleaned up and resupported MSHA and state were notified and they did an investigation.

Found roof fall at 1:00 p.m., 1 break out by spad #3942 in #8 entry on 2 left mains. Fall measured 35 ft. long, 19 ft. wide by 4 ft. thick, approximately 1,800 ft. outby the section. Rotten top was determined as the cause.

A unintentional fall occurred in the main north #8 entry at the 42+50', due to deterioration of the roof above the bolt anchorage. The area had been bolted with 6' tension rebar bolts.

A fall of roof occurred in the 1st Northwest mains in the # 6 Entry at spad 124+43 (x-cut 152 ) approx 3,020' outby. The fall was approx 20' long x 20' wide x 8' high.

The #12 entry had streaks of sandstone & shale in the roof. The area was bolted with 5/8 x 72"" fully grouted rebar & boards. The entry has been discontinued. The fall area is dangered off & timbered.

A fall of roof occurred in the 1st Northwest mains in the # 7 Entry at spad 127+93 (x-cut 157 ) approx 2570' outby. The fall was approx 90' long x 20' wide x 9' high.

Groundfall at the 4080 level. MSHA contacted and inspected.

Rock Fall discovered 105'l x 19 1/2'w x 6-8' high in #2 entry (east mains) 1 brk inby spad 636-Blocked secondary Escapeway-pulled all mcu outby.

A roof fall occurred on the 2 Left, 4 West panel at crosscut 16 in entry #2. The cause of the fall was weak laminated shale commonly referred to a stack rock. The fall was located in the weekly examiners normal route of travel. His route of travel was rerouted around the fall.

An unintentional roof fall occurred in the #2 entry of the 2MW (61+20) at 102 xc of the return. The fall was approximately 40' long x 16'10"" wide & 5 1/2' high.

An unintentional roof fall occurred on the 2nd main west #3 entry between #82 & 83 cross cut. This roof fall measured 25' long x 19' wide x 9' high and the roof fall will be cleaned up and resupported.

Main Portal Elevator inside doors had broken cable. Cable was replaced and elevator was put back in service at 7:20 a.m.

Roof fall in return areas for I-Butt spad 4324 also 4161 E-Butt return 3681 spad and 5917 old rooms both areas.

Elevator would not close properly, because of the retiring cam. Men were brought in and out of the mine by the slope entrance. The elevator was repaired and running by the end of the day shift at 4pm.

This roof fall occured on #9 D belt in #8 entry 2 brks outby spad #37061 fall is approx. 4000 ft out of face. Bolted with 5 ft torque tension bolts and will not be cleaned up fall is approx. 60 ft x 20 ft by 8 ft thick.

A roof fall occurred in the main east from crosscut 6 to 7 in #6 entry. The fall measures approximately 70 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts and truss bolts.

A inactive hole (gas or oil) was intersected on the midnight LW shift at 21 Shield of the 12 East longwall section. No accumulation of methane was found but was liberating 2.6% over the hole. The hole was opened up by using non-sparking tools and plugged. THE OPERATOR IS FILING THIS REPORT IN PROTEST AS MSHA IS SAYING THERE WAS AN INUNDATION OF GAS. NO ACCUMULATION WAS FOUND.

A roof fall was discovered in the 1 west #5 panel, crosscut 7 to 8, #2 entry. The fall measures approximately 30 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts and 10 foot point anchor bolts.

A rock fall occurred at 12+60 in the 3rd east submain in entries 5 & 6. The fall was 90 x 19 x 6 and ran north & south. The fall will be cleaned up.

A fall of roof occurred in the 1st Southeast mains in the #1 Entry at spad 6+30 (crosscut #9) approx 850' outby. The fall was approx 60' long x 20' wide x 10' high.

A roof fall was discovered in the 1 west #5 panel, at 9 crosscut, #3 entry. The fall measures approximately 30 feet long, 18 feet wide, and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts.

A roof fall was found at 12:15 pm on 9/25/2006 by examiner doing a pre-shift. Fall was in four way intersection at 48 cross cut entry one of 5RMS. Area was mined March 2003 and had been supported with 8' full resin bolts. No injury or property damage resulted. There are no current plans to clean up fall.

A fall of roof occurred in the 1st Southwest mains in the #5 Entry at spad 3+50 approx 1,130' outby. The fall was approx 20' long x 20' wide x 10' high.

A fall of roof occurred in the #3 entry of the main north left aircourse. No blockage of air movement or entrapment occurred. This entry is not a normally traveled entry in the old works. The entry has been dangered off.

HOIST WAS DOWN 4:45 AM UNTIL 5:30 AM THE HOIST MALFUNCTIONED DUE.TO A MOTOR ROTOR BRAKE SYSTEM SENSOR

On 09-25-06 at approx 6:55 P.M. on MMU- 003-0 the continuous miner operator was finishing his 3rd lift in the face of # 1 Entry when an orange glow was detected at the head of the miner and approx 3ft to 4ft from the head. The approx size of the glow was basketball size. No injuries resulted from this

A fall of roof occurred in the 1st Northwest mains in the #2 Entry at spad 113+71 to 114+67 (x-cut 138) approx 4,066' outby. The fall was approx 20' long x 18' wide x 8' high.

Roof fall on 1st left off NW mains. 2 breaks inby SS 844. Length-120 X 20 wide- 10' to 8' high. This is a gob area, no main airways were affected.

ROOF FALL (7000-1-016-06) : LOCATION : 1W #3 Belt-Crosscut #109. DIMENSIONS : H-12', W-19', L-25'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 5' fully grouted bolts

On 9/24/06 an unintentional roof fall occurred on the 2nd 42 South Belt #5 entry at #60 crosscut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 25'l x 18'w x 14' H and the roof fall will be cleaned up and resupported.

An unintentional fall of roof occurred on the belt of 002 section break #4.

9/23 & 9/24 with reported 10"" of rain (guage at mine went to 6"") overwhelmed the sump at the drift mine pit and the 8"" electric pump and power went off and the 8"" diesel backup's clutch went out and water ran into the mine inundating it for the 300' it had advanced. Water roofed on left side 4th & 5th xcuts (#1 entry) and water 3000' at 4th xcut in #7 entry and face.

A roof fall occurred on the 2 Left, 4 West panel in entry #2 between crosscuts 43 and 44. The cause of the fall was weak laminated shale (stack rock). The fall was located in some abandoned coal hauler stations adjacent to the section roadway.

A roof fall was discovered at break 6 on B belt line. The estimated size of the roof fall measured 7 ft high and 18 ft width and 50 ft long.

Rock fall was in #3 Entry, 3rd Main East Intake. 15' x 15' x 6' high. 6' fully grouted bolts. The fall was in part of intersection & part in pillar line.

A supervisor walked out of the building and saw in the distance what he initially thought was smoke or a mist which was close to ground level. It was several hundred feet outback of the building. Upon closer investigation, it was determined to be gas spewing up through the ground.

Unintentional roof fall #7 main north entry #7 xcut belt line. 16' long x 21' wide x 5' to 6' high. 3 way intersection bolted with tension rebar bolt, with props and cribs. Cleaned up and rebolted. Date of development Jan 1977.

Unintentional roof fall 2nd west panel #5 entry at 450' mark. 25' long x 25' wide x 12 to 14' high. 4 way intersection bolted with 6' to 8' tension rebar bolts. Area proped off on travelway moved around.

East 2, 25-26 block, 35 entry, MMU 007. 50' long x 18' wide x 8' high. Support: 6 ft 3/4 inch resin bolts, pans straps. Fall dangered off, additional roof support installed around fall.

ROOF FALL (7000-1-015-06) : LOCATION : Main E Return-Crosscut #106, #2 to #3 Entry. DIMENSIONS : H-7', W-29', L-120'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 5' fully grouted bolts.

1/2 Break outby No. 3 Belt Tailpiece, wet rotten conditions caused a section of the roof to fall. The fall is 25 ft. long by 18 ft. wide by 6 to 12 ft. thick approximately 9,000 ft. from the active section.

ROOF FALL (7000-1-014-06) : LOCATION - Main E-Crosscut #118 #1 Entry. DIMENSIONS : H:6', W:19, L:120'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 5' fully grouted bolts.

2nd southwest, station 16+20 crosscut between entry #4.

An unintentional fall of roof occurred while retreat mining was being conducted on 001 section. The fall occurred in the left wall at break #7.

ROOF FALL : (7000-1-013-06) : LOCATION : Main E-Crosscut #120-#121 #3 Entry. DIMENSIONS : H-6', W-19', L-80. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 5' fully grouted bolts.

A roof fall was discovered 1 break inby spad 972 in #3 entry. The estimated size of the roof fall measured 6' high, 18' wide and 20' long. No injuries were reported as a result of this roof fall.

Oldenburg Scaler in service since Jan. 05 caught fire while scaling in S-27 1600' level. The fire was probably caused by a hydraulic leak ingnited by normal operating temps of the engine. The scaler was a total loss.

On 9/21/06 a blast pattern was detonated in the main pit and seconds later the south high wall just above the blast sloughed down. Appoximately 500,000 tons.

ROOF FALL (7000-1-012-06) : LOCATION : Main E-Crosscut #113 Betwen Road and Belt. DIMENSIONS : H-6', W-19', L-50'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 5' fully grounted bolts.

At approximately 12:55 on the North Left section, MMU 003-0, 18 block, x-cut #3 to #2 there was an alledged flash generated from cutting sandstone. The intial investigation by MSHA and the state determined negative findings.

An unintentional fall of roof was discovered on a weekly exam in east-west mains, 1st panel, right break #47. The fall was approx 20 x 20 x 8.

Roof fall, 60' x 19' x 10', Northwest mains #4 entry, spad #937. No injuries, passage not blocked.

Examiner found roof fall at the 1360' in x-cut between #5 & #4 entry of the 2nd east entry along the 1A belt. Fall is 35 - 40' long, 17 - 18' wide, 7 - 8' high. Primary roof support was 5' double lock. Fall has been timbered on both ends. Additional support has been installed on roadway. Fall will not be cleaned up.

Power went out causing hoist to go down at 8:40 A.M. Problem was with electric company (Amerun UE). Hoist was back in service at 11:30 A.M. MSHA was notified of power outage and when power was restored.

An unintentional roof fall occurred at the #32 Crosscut/Flannigan belt. The fall was approximately 20 feet long, 17 feet wide and 15 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up.

Approximately 30 minutes after cutting the #3 right x-cut thru into the #4 entry (@ #25 xc at station spad 1:+91) creating a three (3) way intersection, the intersection of #4 entry fell in. The intersection was bolted with six (6) ft fully grouted roof bolts and less than 4 ft contours. No one was injured, no equipment was damaged.

During the semi-annual NDT testing, the Koepe mancage hoist ropes showed increased wear, enough to warrant changing. The mine was evacuated and ropes will be changed.

Unintentional roof fall occurred in neutral airway 20 x 20 x 8' high was found during pre shift of track 3rd Northwest Mains #5 entry spad #1108. Roof support was 6' torque tension & 8' cable bolts. Wet weather fault.

A rockfall measuring 40'L x 18'W x 5'- 6'""H occurred in in the no. 4 entry of the No 3 NE Mains (No. 9 belt) 140 inby spad No. 5176.

A roof of mine roof occurred on the 005-0 section, 3 right panel, #4 entry at #9 crosscut. The fall was approximately 19' wide, 35' long and 6' thick. The fall occurred during retreat mining, catching the Joy continuous miner and 2 MRS's. The material was removed and the equipment removed safely.

A roof fall occurred in the main west return. The fall was 50' long, 18' wide and 7' high. The fall was between xc 18 and 19 xc and not in the intersections.

A roof fall was discovered in no. 4 entry of 3 Right off 2 East. The fall was approximately 200' long by 4' high by 18' wide. The fall will not be cleaned up.

A reportable roof fall was found in E Seam, 10 Headgate, entry1, crosscut 1. The fall was approximately 10 feet high, 24 feet wide, and 40 feet long.

A fall of roof occurred in the 1st Northwest mains in the # 5x-cut 126 approx 4,280' outby. The fall was approx 26' long x 18' wide x 6' high.

East 2, 17 block, 3 entry, MMU 007, 4-way intersection. 20' long x 20' wide x 8' high. Support: 6' 3/4"" resin bolts, pans and brow straps. Fall dangered off, additional roof support installed around fall.

Roof fall on 2nd west main #1 entry return spad #3584. Also on E-Butt - 3418 spad # and 3530 # old rooms.

A fall of roof occurred in the 1st Northwest panel in the #5 Entry at x-cut 16 approx 650' outby. The fall was approx 20' long x 25' wide x 10' high.

Unintentional roof fall approx. 160' x 20' x 8' fell in old 11 RT (idle) belt entry. Inby spad # 11 R 79.

An ignition occurred at 111+16, #3 Entry, F-14 Section. The ignition lasted approx 60 seconds and was extinguished by the water sprays and wash down hose on the miner. There was no damage to equipment and there were no injuries.

A fall of the roof occurred on the 1st North West submains on #3 unit, #7 entry at x-cut 74. Ventilation, equipment and personnel was not affected. The fall measured 30 x 20 x 5 thick.

A roof fall occurred in the intersection of the #2 heading at break number 36. The fall is 25 feet by 40 feet long.

Unintentional roof fall, #5 entry (neutral) 70'x 18'x8'.

A roof fall was discovered 1 break inby spad 279 in #9 entry, the estimated size of the roof fall measured 5' high X 18' wide and 40' long.

Roof fall found while firebossing the mine. It was in 5 South Mains No.5 & No.6 headings, spad #74 and spad #80. The fall was 80' long, 18' wide, and 5' high.

A roof fall occurred adjacent to the last shield on the face causing the entrance to the tailgate to become blocked, preventing egress off the tailgate end of the longwall.

A roof fall occurred on the Main North Track at 43 x cut, 04 Entry, 40 + 70 feet (old 4 West Track Spur). Fall was 30' long, 20' wide, 5' high. Area was in June, 1989. Fall occurred due to deterioration of the immediate roof around the existing roof bolts. Area was supported with 8' roof bolts, resin anchored 2' expander shells. Plank, hedder block & 12' cable bolts were used.

An unintentional roof fall was discovered on the #1 section, 1 break inby spad station # 5029, from #7 entry thru #9 entry in the last open X-cut. The fall measured 90' long x 17' wide x 11' high. This miner section has been withdrawn and ceased mining.

A roof fall was found at x-cut 76 entry 4,5R main south. Fall was in four way intersection that had been mined October 21, 2004. Area was bolted with 6 & 8 foot full resin pins. No injury or property damage resulted. There are no plans to clean up fall.

A rock rall occured in the 1 main south, #6 entry, in part of the pillar line between crosscut #5 and #6. The fall measured approximately 20'x19'x6'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.

A roof fall occurred in the main east, crosscut #37, between the belt and travelway. The fall was approx. 18' long, 20' wide and 6' tall. The area was bolted with 6' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.

The hoist was being prepared to pull a continuous miner out of the slope from the under ground.The brake car was in the side yard switch chained off. While pulling the hoist to double sheave the rope, slack developed on the hoist drum causing the rope to tanle and due to the tangled rope management at that time decided to change out hoist rope. The hoist was down from 9:14-5:02 pm.

A roof fall was found on 9/16/2006 at x-cut 160 entry 1 main south. Fall was in four way intersection that had been mined December 11, 1996. Area was bolted with 6 and 8 foot full resin pins. No injury or property damage resulted. There are no plans to clean up fall.

Roof fall between spad no. 380 and the belt entry approximately 80 feet length and 20 feet width. No injuries.

Unintentional roof fall occurred in intake airway, not affecting escapeway 20' long x 19' wide x 8' high in intersection, was found while traveling weekly in 2nd Northwest Mains #8 entry spad #351. Roof support was 6' resin. Wet weather slip.

A fire was observed behind the cab of the bulk emullsion truck. The truck was fully involved within minutes. The blast pattern was evacuated and approximately 70 minutes after the fire started an explosion occurred. The bulk emullsion truck was a total loss.

Roof fall - F entry, 2.5 South @ SS 7 + 27. Approx 20 ft long, approx 25 ft wide, 8 to 10 ft high. Grey shale.

The man hoist would not operate, trouble shooting found several fuses blown. There was a thunderstorm in the area, possibly a current spike blew the fuses. The hoist was out of service for 35 minutes. Two other man hoists were operable.

A roof fall occurred in the 1 west #3 mains at 63 crosscut in #5 entry. The roof fall measures approx. 15 foot long 18 foot wide and approx. 6 foot in thickness. the area was supported with 5 foot fully grouted bolts.

A roof fall occurred 10'-15' inby survey station #6023 in the No. 3 entry on the 003-0 section. The fall measured approx 40' long x 20' w x 8' thick. The area had been bolted with 48"" resin bolts & supplemented with 12' cable bolts. The fall occurred in an area of known bad roof & began working on the 3rd shift. The area was dangered off. The fall occurred at approx 6:35 am.

East mains 2, 29 block, 10-11 crosscut, mmu 004-0 18'w x 40' l x 8' h. Support: 6'3/4"" resin bolts, pans, straps. Fall dangered off, additional roof supplor installed around fall.

A roof fall occured in the 4 East panel, #8 entry, intersection of crosscut #58. The fall measured approximately 19'x19'x7'-9'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.

ROOF FALL (7000-1-011-06) : LOCATION-Main E, crosscut #115, entry #2. DIMENSIONS-H:6', W:19', L:50'. GEOLOGY-Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT-5' Fully grouted bolts.

A roof fall measuring 40' long - 15' wide and 6' high fell between blocks at break 42. The roof fall consisted of lamenated shell. The area was timbered and cribbed and reported to MSHA.

An un-intentional roof fall occurred in the intersection of number 6 entry on the working section mmu-001, one break inby the section loading point. The roof fall measured approximately 30'feet in length x 18'feet wide x 5' to 5 1/2' feet high.

A roof fall occurred in the main west, crosscut #38, entry #4 station #3080'. The fall was approximately 18' long, 18' wide, and 8' tall. The area was bolted with 6' & 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.

A maintenance worker and contractor were trapped in an exterior elevator on the silos when the elevator lost power. After reviewing emergency lowering procedures with the manufacturer, the elevator was lowered to the ground.

An unintentional roof fall was discovered at 10:00am on 9/14/06, during a weekly examination. The roof fall material consisted of laminated slate and sand stone. The fall measured approx 20' wide X 16' long X 4 1/2 - 5"" high.

A roof fall occurred in the intersection of the #2 heading at #43 crosscut. The fall size was 30 foot by 40 foot.

An unintentional roof fall approximately 100' x 18' x 6-10' occurred on the return near the ""Dead Mens Branch"" bleeder fan.

A train was assigned to pick up some hot lignite out of A1 silo at the Beckville mine. When train operator began to fill the first car the lignite dust flashed and then went out. The operator continued to load until he had emptied the silo (five cars). The hot lignite was taken to the power plant where it was used.

Roof fall in 6xR X-cut 1 X-cut outby spad # 2571. 30' x 20' x 4'. No injuries

Hoistman bringing N Skip to dump & S Skip to load, noticed over travel limits kicked in. S Skip dropped fast & he noticed slack on N rope. Factory welds on N Skip broke loose from the top deck that connects to the rope and N skip fell down the shaft.

A roof fall occurred in #5 Entry at #57 wall 4 way intersection, #3 1 North Mother Belt. The fall was above anchorage. There was no damage to equipment. The fall was 20' wide, 16' high, and 50' in length.

Roof fall in spad 959 60' outby in #1 heading. Measured roughly 15' high, 12' wide, 40' long. Horseback slip fell out 8 feet of rock.

A roof fall was found at no. 2 heading at spad no. 463 measuring 90' long, 18 - 20 feet wide and 12' in height.

A frictional methane ignition occurred at shield No. 37 in the Gil-5 longwall panel. The crew was cutting from the headgate to the tailgate at the time of the ignition. The Headgate shearer operator stated that he bumped the roof with the bits and noticed the flame between the drum and the roof. The flame lasted 3-4 seconds.

Cedar Creek elevator is out of service as of 9/13/06 due to electrical problems. Bagby Elevator Company examined the service elevator, but did not have the part needed to repair it. We are presently still in search of part to repair elevator.

A roof fall occurred one crosscut outby #5 seal of the 2nd main north seals. The fall was approximately 22' wide, 22' long and 8' tall. The area was bolted with 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.

An unintentional roof fall was discovered while pre-shifting a beltline. A stopping was found crushed out and after further investigation the roof fall was detected. It is not known when the fall occurred. The lenght of the fall was approximately 40 long, 20 feet wide, and app. 12 feet thick.

A roof fall occurred on MMU-001 in the last open crosscut in entry #2. The cause of the fall was weak laminated shale. The fall was timbered out then after modification of the K-order clean-up and rehabilitation work commenced.

At spad 473 in return airway, a fall was found while return airway was being made.

A roof fall has occurred in the intersection of L Main entry #6 at spad 57 + 38. The fall measured 21' long x 19' wide and approx 8' thick.

The unintentional roof fall occurred between crosscuts 157 and 158 in the Galatia North travelway. The fall was approx 15' long, approx 8' wide and approx 6' thick. The fall did not affetct ventilation or passage of men. The fall will be cleaned up.

Roof fall approx. 1100"" down slope from portal causing damage to steel roof supports. Roof was supported by steel sets 24' wide consisting of 12 X 65 horizontal beams and 2 ea 10 X 54 vertical legs, w/4"" x 6"" wood lagged and cribbed w/ 6"" hardwood.

The intersection one bock inby spad #1080 in #8 entry fell 5' high x 18' long x 20' wide. The area was bolted with 5/8 x 60"" full grouted rebar. No known remove for this fall. Area dangered off, set breakers, and will not be cleaned up.

Found roof fall when examining. Roof fall is at 8201' of the #1 entry main north entry. Roof fall is approx 90' long, 17-18' wide, 6-7' high. Fall is in intersection. Fall has been timbered & cribbed off & will not be cleaned up. Primary roof support was 5' double lock bolts.

As driver was passing another truck when it started stalling out and started moving backwards. The driver jumped out as the loaded truck went over an embankment. The driver was uninjured.

An unintentional roof fall occurred 6200' outby the face at Break #99, #7 Entry, 1 Break inby Spad #1349. The fall measured approx 12' long X 20' wide X 4 1/2' high.

On 9/11/06 an unintentional roof fall occurred in the last open cross cut #9 entry, #39XC. This area was supported with 6' foot point anchor roof bolts. The roof fall measured 70' l x 18' w and &7' in thickness. The roof fall will not be cleaned up. All approaches to this fall have been cribbed off.

A roof fall occured in the 1st South in #7 entry between 17+60 and 18+40 measuring 45'x 20'x 6'. The area was mined in August 2005 and supported with 5' fully grouted roof bolts. The fall was caused by a hidden slip.

A pillar fall occurred after taking the final lift from the No.8 block on the 3-right off No.1 Northwest panel (3 breaks inby survey station No. 5274). The fall disable the #4 mrs location in the #9 entry & caused the #2 mrs to go to solid preventing it from being moored.The mrf were recovered.

An unintentional roof fall was discovered in the #8 entry, #36 break of #4 belt, one break outby spad #3752. The fall measured 50' h x 18'w x 5'h.

A rockfall occurred @ 1pm on the #2 Unit in the 3rd West Panel. The fall blocked our secondary escapeway. The fall was between #3 & #4 entries, extending to #5 entry. The fall was 10x19x9. Msha was notified within 15 minutes.

Main Portal Elevator down on ""overtravel."" Armature problem on MG set. Repairs were made and elevator was put back in service at 8:50 pm.

Rock fall occurred in 1 North Mains in #2 entry at spad 711 between break 33 and 34 in the return. Fall measured approx. 20' wide x 50' long x 3'-6' thick.

A roof fall approx 150'L X 20' W X 6'Thick was discovered 4000 ft. outby the face. It is located in the #1 entry 30 ft. outby spad station #420.

A roofall measuring 75'L x 20'W x 8'H occured 3 breaks inby spad number 8331 in the 4th LT Panel, 001 section (002 MMU). Area was bolted with 6' TT bolts.

A roof fall occured on #3 Unit #7 Entry crosscut #11 at spad 6+75. The fall was approx. 20 x 30 x 6-8' high.

1st Right Section, crosscut 30 in the #2 entry, on Thursday 9-7-06 an examiner encountered a broken timber on the inby right corner and reported it to mine management. Mine Management placed additional roof support in intersection. Total of 15 cribs were in the intersection prior to the incident. The roof conditions continued to worsen resulting in a roof fall.

ROOF FALL (7000-1-010-06) : LOCATION-Main E #5, crosscut #1 belt entry. DIMENSIONS-H:8', W:19', L:60'. GEOLOGY-Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT-5' Fully grouted bolts.

An unintentional roof fall was discovered on 9-10-06 at approx. 9:00 PM while pre-shift exam was being conducted. The fall occurred sometime during the weekend, time unknown. It measured approx. 20' wide x 20' long x 8' thick in the #2 entry, last open x-cut. The fall appears to be a result of weak overlying strata that has deteriorated.

A roof fall occured in the 1st Southeast Main spad #0+70 #6 Entry. The fall was approximately 18' long x 18' wide x 10' high.

A roof fall occured in the 1st Southeast Mains spad 0+70 #6 entry #1 Unit Return. The fall was approximately 20' long by 18' wide by 10' high.

A roof fall occured in the 4 East panel, crosscut #48, starting in #1 right and ending in the middle of the intersection of #3 entry. The fall measured approximately 100'x19'x6'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.

A roof fall occurred in the 1 west #3 mains at 60 crosscut in #5 entry (travelroad). The roof fall measures approx 30' long, 18' wide and is approx 8' thick. The area is supported with 5' fully grouted roof bolts.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #6 (RETURN) ENTRY OF THE 2ND WEST SECTION BETWEEN X-CUT 30 AND X-CUT 31. THE FALL MEASURES APPROXIMATELY 120' L x 16' W x 9' H. CRIBS AND TIMBERS ARE BEING SET.

An unintentional roof fall occurred on the 1st Main South at the 15 xc (9+00) in the #7 Entry. The fall was approximately 40' long x 18' wide x 5' high. The fall was found during the weekly examination and reported to an inspector on site.

ROOF FALL (7000-1-009-06) : LOCATION - Main E #5, crosscut #1 between road and belt. DIMENSIONS - H:6', W:19', L:50'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' Fully grouted bolts.

On 9/8/06, an unintentional roof fall occurred in the #1 unit #3 entry at #85 cross cut in the last open crosscut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 35'long x 18' wide x 10' high and the roof fall will not be cleaned up and resupported.

An unintentional roof fall was discovered outby the #1 section between entries #10 and #11, across from spad station #4982. The fall measured 45' long x 18' wide x 10' high. This fall was outby the working section.

Doors would not stay closed on Main Portal Elevator due to 4-inch wire tie broken. Repairs were made and elevator was put back in service at 3:30 a.m.

A roof fall occured in the 15 North panel, #4 entry, in the pillar line between crosscuts #16 and #17. The fall measured approximately 35'x19'x7'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.

Roof fall in No.8 Heading in 5 South Mains at Spad #1022

The intersection 1 block inby spad #1677 fell 18' long x 40' wide x 12' high due to wet roof bad geology & subsidence. The area was bolted with 68x72"" rebar with 6x6 plates + 2x8x14"" boards. The area had been spot bolted with 12' cable bolts. The fall will not be cleaned up. All sides cribbed & permanently dangered off.

During pre-shift, found roof fall at break 68 & 69 outby spad 809. Fall 6x20x40.

at 9:15 am the hoist inspection was being donw when the inspector noticed two. Certified engineer and certified welders were called into make required repairs. The mine was evacuated once the cracks were found. Evacuation through our seoncdway out at one no. 2 mine shaft.

On 9/7/06 an unintentional roof fall occurred on the #2 Unit between #6 to #7 Cross Cut in # 3 entry. This roof fall was supported with 10'point anchor roof bolts. The roof fall measured 30' long x 20' wide x 8' high and the roof fall will not be cleaned up and resupported.

An unintentional roof fall occurred in the intersection at break #85. The fall measured approximately 8 - 9 ft. high x 20 ft. wide x 30 ft. long.

The fall occurred last evening on unit 1 around 6:30 pm. Fall occurred in an intersection in #6 entry one break outby spad #39563. Area had been cribbed & rope bolted & was bolted with 5' torque tension bolts. Fall was approx 20' x 20' x 8' high. Additional cribs have been installed on the belt line & all approaches have been cribbed off. The fall will not be cleaned up.

Roof fall above anchorage was found during normal examinations of the bleeder system. This fall does not impeed travel of air or men. All entrances have been breakered off and area will be rehabilitated. Measurements: length 80', width 18' to 20', height 6' to 8'.

Roof fall-E-entry 21/200@ ss10+55. Approx 45' l, 38'w, and 8' high in grey shale.

A roof fall occured in the 4 east panel, #7 entry, intersection of crosscut #13. The fall measured approximately 20'X20'X6'-9'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.

Production skip overtravel. While waiting for the release from high surge bin on surface to resume the hoisting cycle, the loaded west skip began to roll back and came to rest when the east skip encountered the crash beam below the hoist drum. Damage was confined to bent handrail on the east skip and 5 broken wooden dividing timbers below the west skip.

A roof fall above anchorage occured 7 N section #1 entry @ 27 block.

A roof fall occured on #1 unit #3 entry at spad #4+20 The fall was approximately 30' long x 20' wide x unknown height.

A roof fall occurred in the 1 west #5 panel, #8 entry, 31 crosscut. The fall measures approx. 20 ft long, 12 ft wide, and 8 ft thick. The area is supported with 4 foot fully grouted resin bolts, and 10 foot point anchor bolts.

A roof fall occurred in the 2 North #6 Mains, 72 crosscut, #4 entry. The fall measures approximately 25 feet long, 18 feet wide, and 10 feet thick. The area is supported with 5 foot fully grouted bolts and 10 foot point anchor bolts.

Rock fall break #54, 6' to 7' high, 19' wide, 20' long in #6 entry 300' from face.

A frictional methane ignition occurred on the Gil-5 longwall face. The crew was in the process of cutting toward the headgate when the tailgate shearer operator noticed a small orange glow at the bottom of the tailgate drum. He estimated the flame lasting approximately 5 seconds.

During normal operations, main hoist stoped operating, due to fault code in the direct drive system. Reset direct drive system and hoist is operating fine.

A roof fall was discovered in the 2 south #2 mains, 38 crosscut #3 entry. The fall measures approx 40'long, 18' wide, 10' thick. The area is supported with 5' fully grouted bolts and 10' point anchor bolts.

North 6, 13 block 7 entry, MMU 004, 4 way 18ft. wide 18ft. long x 8 ft. high. Support 6ft. 3/4 inch resin bolts, 12ft. cable bolts, pans, brow straps, truss bolts, fall dangered off additional roof support installed around fall.

An unintentional roof fall occurred in the #3 intersection at the break inby Spad #2793. The fall measured approx 8'-10' high X 20' wide X 40' long. The fall also covered approx 2/3 of a Fletcher roof bolt machine, as described below.

A roof fall occurred on the 80 section. The fall occurred inby the tail piece.

A roof fall measuring 60' long 18' wide 6' thick was discovered in the return entry spad 21542. Fall was 11,800' from the active section in an area developed 02/97 with 5' resin bolts. Neither ventilation nor the passage of men was affected, no injuries or equipment damage occurred from the fall. The perimeter of the fall was re supported with timbers and barricaded.

An unintentional roof fall occurred at SS#3505 in the #8 entry of 2-right mains. The fall dimensions were approx 40'l x 20'w x 7' high.

SSX Mine Z1-6945-XC3. Operator was bolting up the area when he noticed the ground starting to work. He called his supervisor and told him that he was going to pull out because of the ground working and they need to shotcrete this area before they could bolt it. Operator was pulling out of area and stopped to roll water hose up, when the back came in on the Boom of the bolter.

An unplanned roof fall was discovered at 34 cross cuts inby the portal on the #1 belt. This mine is in non producing status. Time of fall is unknown. Fall measured approximately 25' long x 30' wide x 10' high.

Unintentional roof fall 20'(L)x 19'(W)x 10'(T)- two breaks outby spad 15822.

A roof fall was found in the intake entry 5,200' from the active section. The fall measured 65' long 18' wide 4' thick. 4' resin roof bolts were in place. Ventilation and the passage of men were not affected, no injuries or equipment damages occurred as a result. Perimeter of the fall was resupported with timbers and barricaded

Rock fall in No. 7 entry five breaks outby #3 Head drive on #2 mains. Rock fall was 8 ft long, 20 ft wide and 5 ft thick. It was bolted with 4' resin bolts. Crib off.

Rock fall in #6 entry 10 breaks outby #5 Head drive on 4 mains. Rock fall was 140 ft long, 20 ft wide and 12 ft thick. Crib off.

Roof fall was found when examining entries in 1st east. Fall is at the 320' of the 1st east entries between #1 & #2 entry. Fall is 45-50' long, 17-18' wide 7-8' high. Fall is in pillar line. Fall has been timbered off & will not be cleaned up. Primary roof support was a 5' double lock.

A roof fall occurred in the 2nd west submain, #2 entry, #1 crosscut, 70' station. The fall was approx. 20' long, 20' wide and 8' tall. The area was bolted with 6' & 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.

A fall occured on #1 unit #6 entry at spad 4+90. The fall is approximately 20' long x 18' wide x 6+' high

A rock fall occurred on 1 East Mains at break 22 between #3 and #4 headings. Fall measured approx. 20' wide x 20' long x 3'-4' thick.

The roof fall was in an active area of the mine where benching was taking place. This fall occurred over a long holiday weekend, there were no personal injuries or equipment damage. The fall did not impede ventilation or passage. The report time was within the required 15 minutes. Location-16 east x 33 south. Size-aprox. 50' x 50' x 11'. It was above the roof bolt anchorage zone.

On 9/4/06 an unintentional roof fall occurred on the 1st 42, North Belt line, #4 entry between #52xc & #53xc. This area was supported with 6 foot point anchor roof bolts. The roof fall measured 45 feet in length by 18' feet in width and 8 feet in thickness. The roof fall will be cleaned up and resupported.

A roof fall was discovered in the 2 south #2 mains, crosscut 40, #6 entry. The fall measures approximately 25 feet long, 18 feet wide, and 7 feet thick. The area is supported with 5 foot fully grouted resin bolts and 10 foot point anchor bolts.

A roof fall was discovered in # 5 entry 1 break inby spad 605 the estimated size of roof fall is 6ft high 18ft wide and 40ft long. No injuries reported.

A roof fall occured that was approximately 22' long x 18' wide x 8' high

At approximately 8:50 AM a frictional methane ignition occurred on the Gil-5 Longwall, at shield No. 15. The flame was described as a small orange glow at the bottom of the cutter drum. The flame lasted one to two seconds before it was extinguished by the cutter drum water sprays.

A fall occured in the 1ST Northwest mains #2 entry crosscut 163 approxaimately 30' long x 18.5' wide x 6-8' hight

Rock fall break #29 on 3rd left. 3.5' to 5.5' high, 50' long, 19' wide 2,430 ft from face #6 entry.

Brake car stuck on slope bottom. Emergency stop light lit in control room. Brake car was back in service at 12:50 a.m.

An unintentional fall of roof was discovered on high top panel break #13 #4 entry that was approximately 50' x 20' x 5-6'.

An unintentional roof fall was discovered on east-west mains #1 entry break #2 that was approx 120'x20'x4-7' . The area was cribbed off and will not be moved.

Miner was making cut into face for second shuttle car load when an ignition occured. There were no injuries.

A roof fall occurred on the 3 Left, 4 West panel in entry #5. The cause of the fall was weak laminated shale. The fall blocked the travelway for #5 unit, which was rerouted around the fall.

An unintentional roof fall occurred in the G-6 section #2 entry just inby x-cut 17. The fall was approx. 45' in length beginning at the intersection edge and continuing inby and tapered from 0 to 16' wide. Height at the peak is estimated to be 8-9 ft. A fault ran diagonally across the entry with the edge of the fall being at the slickenside of the fault.

A roof fall measuring 30' x 18' x 4 1/2' occurred in the cross-cut the #4 and #5 entries approx. 7,500 ft outby the section. This would be between our #4 belt and #4 roadway. The fall was reported to the hotline.

Employee was lubricating the slide cylinders with an oil can that operates the mold box on the salt press. His left hand became crushed when the mold box raised and caught his left hand betwee n the mold box and the header plate.

Bin conveyor toppled over while employees were lifting off of tractor, landing on employee's left leg.

Employee was aligning pump motor into position for installation into continuous miner. Pump motor became lodged and as he repositioned motor it shifted catching his right ring finger between motor and frame, fracturing it.

Checking threads on drill rods while head was in motion. Employee put his hands in the tower of drill accidently and was winched by a pulley, causing partial amputation of middle finger and breaking adjoining fingers.

Removing drive shaft from R50 truck. Dropped drive shaft while finger was between joints.

EE was carrying roll of plastic to bag inserter machine, and felt pain in lower back.

The injured employee was helping to clear material from a jam in the crusher. Co-worker (uninjured) pulled a large rock from the crusher, causing him to stumble backwards; he miss-handed the rock to the victim and the force of the rock stripped the skin and tissue off the side of his left hand and little finger.

EE was splitting a block of standstone using a 12 lb sledge hammer and stone chisel when the hammer glanced off the chisel head and struck his left hand resulting in a contusion and abrasion at the base of his left index finger.

Fracture to right index finger: Employee was handing a piece of metal to co-worker near magnet. Finger was smashed between metal and magnet.

Employee was unloading oil at fan in E-15 section when he stepped in a rut and twisted his right knee. Employee began missing on 1/25/07 for surgery to torn ACL of the right knee.

Employee stated that while driving up a hill, the truck stalled. The truck then drifted back into a ditch, turning over on its side.

EE was walking to his shuttle car, when he stepped in a hole and twisted his left ankle.

Employee stated that he was operating a loader in the strip pit when the loader bucket struck the highwall causing loader to stop abruptly. This jarred the employee causing low back pain. Not reportable until lost time incurred on 10-24-06.

EE sat quickly down in boat and he didn't say he got hurt, said he was fine. He wouldn't go to the doctor that day.

Employee was driving mantrip when he lost control of mantrip and ran into the north rib. His left knee came in contact with the dashboard, causing a laceration.

Slipped and fell striking knee on mine floor.

EE was pulling a bent roof bolt out of the top before the glue had time to set up when he felt pain in his lower back. He did not start missing work until 10-3-06.

He was setting up mining in #5 Entry, and grabbed miner cable with one hand and threw cable against rib, Felt pain in lower back.

Working on dozer on shop, rail mashed foot. EE waited around and then decided to go to doctor.

Loading of employee's truck occurred at around 6:20 pm the first bucket contained a large chunk which impacted the truck box. Employee stated he felt numbness radiating from left shoulder to left elbow due to severe jolt caused by the chunk. Received narrative from physician on 10/8/06.

Employee alleges he hurt his right shoulder while picking up cable out of roadway. This incident is under investigation.

EE hit a bump while running scraper and felt pain in upper back near his shoulder. There was a stinging sensation when he would look to his right.

LUBE MAN WAS GREASING CRUSHER WHEN HE SLIPPED OFF THE DECKING AND FELL OVER THE HANDRAIL LANDING ON HIS BACK APPROXIMATELY 4 FEET BELOW ON THE GROUND

Cleaning on #14 belt, picked up a full shovel of mud and coal, felt pain in lower right side of back.

Employee was operating roof drill installing roof bolts. while hauling roof bolt, piece of cap rock fell. As worker was moving out of way He bumped control lever which caused head to move and pinch finger between bit wrench and roof bolter.

Employee was lifting material when he sustained a back strain.

Driving a pin into a cutter chain when a piece of the pin shattered and lodged in his abdomen.

EE was straightening on 8' TR bolt in bolt hole and hyperextended his left thumb. Started missing on 1/4/07.

EE was unloading scoop outside that had water line on it, when he stepped on a piece of hiline cable laying on ground-twisted his right ankle. Went to dr an dmissed 1 shift.

EE was standing beside shuttle car when roof rock fell from between bolt and rib, striking his right shoulder. EE missed work on 9/30/06.

Employee slipped on steering fluid as he climbed the ladder.

Was straighting out roof bolt to install, pinched end of little finger between bolter and bolter head.

Lifting a piece of metal and hurt lower back.

Employee slipped on wet step while crossing car to empty car.

Employee was walking in a drift and twisted his knee after stepping on a rock. Dr. surgically repaired the damaged knee.

Employee was working from a scaling basket on a hammer drill that was lodged in the face. He was having to reach in an awkward position to loosen bolts on the drill when he strained his back. The diagnosis is an L5 disc that is damaged and needs to be surgically repaired.

Employee was running a roof bolter and was in the process of raising up from his knees when he pushed himself up by placing his hand on the boom of the bolter. At this time his knee hit the joy stick on the bolter causing the boom to lower and mash his fingers. This resulted in a laceration to 2 fingers on his right hand.

Employee was getting off dozer, when he stepped on a rock and slipped and broke his foot.

Operator of the 535 HT reported that her bed was bouncing up and down when empty resulting in neck and back pain.

The employee was cutting with a torch a floor grating which he thought was going to fall in one direction, which it did, but it sprang up causing an injury on his left arm.

EE jammed his head into a scoop canopy while hauling block. He had run over a track rail when this occurred.

Employee came down off of the stacker ramp and while walking towards the work truck he slipped on a rock that was lying on the ground near the stacker belt and sprained his knee.

Picking up parts in warehouse, lifting caused strain in stomach - had hernia surgery. Started missing on 1/22/07. Reason for delay in reporting - waiting on medical from doctors.

Employee was attempting to move a screen by pulling it and strained his lower back.

Employee was lowering a steel Hammer plate (side wall of a hammer tool for scaler boom) to the ground off of lift forks and was trying to stand it up at the same time. The steel fell over on upper part his right foot, breaking his big toe's metatarsal bone (up toward the ankle).

Employee was walking down stairs when she missed the last step and twisted rt. ankle, causing sprain.

Installing new bearing on CMI, bearing fell off shaft and landed on his right hand.

Employee was stepping over pipe fittings between the rib and the mantrip when his foot slipped and he fell into the mantrip. Reportable lost time injury 10/6/06.

Ee was stepping off a barge after taking a sample of the sand and twisted his left ankle. Broken ankle.

Employee ws walking in the mill building near the primary ball mill and slipped on the wet floor breaking his left ankle. Building under construction and doors not on - rain water and mud on floor.

Walking down belts & tripped over roller, hit forehead on structure.

Employee was having trouble dumping the 570 haul truck out on the King 6170 high grade dump. Another operator came and helped the employee dump the haul truck. While dumping the load the employee was jarred.

Employee was carrying blocking (3 ft timber) when he fell into a hole, dropping the blocking on his right middle finger.

EE stated that he was leaning over his tool box looking for a splice kit for the continuous miner. EE then proceeded to move a piece of metal out of the way, when he felt a burning sensation in his lower back and left leg.

Employee was cutting insert from CM. Hot slag burnt through pant leg, right side, fell down to top of boot causing burn on side of leg. Did not see Dr. until 10-2-06 and began missing work.

While attempting to carry a crib block under a rock truck, employee fell on the crib block while still holding it causing a fracture to small bones in his hand.

While walking, stepped on a rock and twisted his right ankle and fractured his right 2nd toe.

Employee turned to throw his jumper cable when he twisted his left knee.

Employee was installing a belt wiper. He slipped catching himself, but striking his right shoulder - breaking his right clavical.

Employee stated that he was operating a dozer pushing out shot rock. The right front corner bit of the dozer blade slipped out of the tight rock causing it to jar the employee in the cab resulting in pain in back area.

While roof bolting, Employee was installing bolt, Bolt missed hole, bent and caught finger on steel guide.

EE was performing pre-op on a Lo-Trac. He was standing between equipment and rib, when a piece of middle man rock fell and struck EE on back of head and neck.

Employee was opening a set of metal air lock doors he had placed his left hand at the end of the door and was pulling on it to open when the other door started open and caught his fiinger between the doors resulting in some skin beening deprive from his finger.

He pulled a muscle in right arm while using a hammer to tighten bolts.

Employee was exiting a dozer on the left side, as he stepped onto the step on the push arm (right foot was to the side of the step) he put weight onto his right foot and heard a pop along with feeling a sharp pain. He could not put his weight onto his right foot.

EE was helping 3 others carry a machine door when EE tripped and dopped the door on his foot.

Employee was helping to move a pump. Stepped backwards and tripped, hurting back.

Employee was working on mobile bridge inspection cart and advancing the cart by hand with the aid of three other employees. Low hanging obstruction was approached and employee was struck in the upper torso causing a contusion.

Employee was in process of changing the y-pin on 992G cat loader when the tool that presses the seal in place fell and trapped his thumb between the loader tire and the press tool. This resulted in a mashed thumb and employee had outpatient surgery for damage to thumb on right hand. (Worked two days then took off).

EE was operating roof bolter when a rock fell from overhead, striking him on the right forearm, causing a laceration that required stitches.

When starting to fuel, the nozzle separated from the hose. He jumped back to avoid the whipping hose & fell in the shot rock. He got fuel in his eyes & on his clothes. He struck his leg, knee, buttocks, hip, neck & head against the rock. He retrieved the hose & went to the maintenance area where he flushed his eyes, changed shirt, repaired hose & continued his shift until 7 am.

Employee was assisting with belt move. Tailpiece shifted and fell off crib blocks causing employee to react quickly and fall as he was getting out of the way.

Employee reported: While bolting roof at face, a piece of draw rock fell striking the roof of his canopy. The rock broke; a small section of the broken rock fell from the canopy roof and struck his left leg, sliding down the leg to the top of his work boot.

Employee noticed debris under the guard; he raised the guard to remove the piece of debris and finger was pinched/cut.

Stepped in hole or on rock. Fell to ground. Fracture top of left foot.

On the morning of 9/25/06 EE reported at the end of his shift that he was having pain in his lower back. When ask if he had done anything to hurt his back. He stated that he had not, that the problem had been coming on for some time. When ask if he knew of any specific event that he was aware of to injure his back, he stated, that he did not know of anything.

Miner was helping to load belt structure, tripped and fell backwards on a piece of structure.

Employee was riding mantrip buggy from the face to the track when a piece of belt came in contact with his left knee/leg. Injury resulted in a fracture.

While swinging 20 # hammer to split stone, felt pain - lumbar strain.

Employee was straightening the step on the right side of the loader at the hydraulic tank. He had the boom from the mechanic truck hooked to the step and was pulling the step to straighten it when the step broke. It flew back and hit his chin.

Employee was helping change a shuttle car tire when the tire fell over striking employee on left knee.

Rib rolled and hit the back of his left knee knocking him into the shuttle car.Fractured knee cap, surgery required.

While setting up to cut the straight of #3 entry at 17+40, the miner operator backed the miner out to sight it in when a rock measuring 2'x1'x4"" fell from the roof striking the back of his left hand which was on the remote box for the miner (the rock also broke the box's handle). Initial x-ray indicated bruising only; follow-up determined that a bone in the wrist was fractured.

HE WAS WALKING AROUND 3- WHEELER WHEN HE TWISTED LEFT ANKLE AND WENT TO CATCH BALANCE, TWISTED LEFT KNEE.

Employee was helping to install a culvert at a crossing. He was adjusting chain and backhoe operator moved the bucket. Hand caught.

Employee was exiting Finish Mill # 1. As he stepped down onto platform he twisted his knee. Employee who is a supervisor went to company doctor who released him for work, follow up showed torn medial meniscas in right knee. Employee had surgery on 11/02/2006 making this a reportable lost time accident

Was walking back to his backhoe to move it off the edge of the cut because it was raining. Ground was wet and employee slipped and fell twisting his left foot and spraining his left ankle.

Employee was operating a 50ton haul truck and had started dumping a load of sand when the haul truck leaned to one side at which time the operator retracted the bed of the truck which caused the truck to jerk. The operator complained of soreness to his back and shoulder.

HE WAS INSTALLING LIDS ON GETMAN, WHEN HE STRAINED HIS GROIN, RIGHT SIDE. * STARTED MISSING WORK ON 10-19-06*

9/25 unclogging hopper on screen plant, removing (2) pieces of conglomerate. These pieces dropped down to pile of screened rock. Went to move conglomerate from pile; felt pull in lower back. 9/26 Wednesday morning experienced severe pain & reported to work & left to report to the office. Dr. appt. was made for that morning.

Soil conditions were damp/wet which contributed to employee slipping and falling. Employee struck his back, sustaining a soft tissue bruise.

Employee was walking around the helper side of the roof bolter and slipped on a rock.

Employee twisted around while underground pulling muscle in his back & neck.

Employee slipped on a rubber mat twisting his right knee; did not miss work until 1/18/07.

Employee had been working on a haul truck, and when he dismounted the truck, he felt a pop in his hand. ONGOING He was seen by personal physician, referred to ortho clinic, and notified company he would be off work until approx. 11/3/06

While reclaiming water line employee was taking line down across an underpass when one end of the water line came loose, flew up and struck employee on the mouth. The employee received 7 sutures and a broken tooth.

Standing on cover/guard of tail pulley, straps holding cover broke. Cover not designed to stand on. Employee fell approx. 12 ft to belly pan. Twisted ankle, bruised arm. Employee had fall protection harness, was not tied off, attempting to reach out and hook safety hook, was not 100% tied off. Cover fastened with plastic cable ties that broke.

Employees were cleaning screen house spillage. They opened a floor panel to drop stone into bins. The injured employee walked up onto the stone pile which started to slide. Employee could not stop himself from sliding. The stone and employee slid into the the bin. He fell approx 20 ft onto stone in bin.

Steping from tug to dredge he slipped and fell down injuring his shoulder, while off shift.

The HWM electrician developed pain in his lower back after his right foot slipped as he stepped into the power head area of the Superior High Wall Miner. He finished his shift and started off on 09/24/06.

Driver was operating a haul truck on the upper haul road. He hit a compression bottoming out the vehicle, causing injury to his lower back.

Employee was putting a steering gear box in the lube truck when he felt a sharp pain in his right shoulder. He was taken to seek medical attention and is going back to get a work release. On the Oct 11th he was still having pain he was sent to see an orthopedic surgeon the following week and was taken off work until his shoulder heals.

Getting out of crew vehicle at end of shift w/ lunchbox in hand when she tripped & fell to concrete hitting her right knee.

Employee lost control of his jackleg drill as the leg extended. He let go of it and put his right hand on the rib as he turned to get out of the way. The drill fell to the side and struck his right, fifth finger. It broke the finger at the second (PIP) joint.

Injured was removing material from a stopped up air pump. He had turned the pump off and stuck his hand down inside of the pump; as he was doing this his knee struck the cut off valve for the air and turned the air on to the pump. This caused the pump piston to move and strike his hand and pinch it between the pump housing and the piston.

While changing brake pads on a rock truck company #87. brake caliber fell smashing middle finger on left hand requiring stitches.

Employee was standing on the car loading side of the launch. The miner was starting into hole #738. Injured stated that he took a step backwards to regain his balance when he fell about five feet to the ground, landing on his left shoulder.

Employee was assisting in cleaning concrete out of a pipe. When the air line was turned on, the shut-off valve broke and the hose was whipping around. The end of the hose struck employee on the inside of left knee.

Employee was moving a stone slab, when a piece of the slab broke and cut employee's finger.

Employee was attempting to warm his right hand on a heater fan in a dozer when the fan contacted his middle and ring fingers causing laceration injuries requiring sutures.

Roof bolter operator was drilling roof in x-cut #2 to #1. The steels clogged up so he removed them from the roof. He struck the starter steel on the boom of the bolter. The mud in the steel blew out the back end of the steel striking him in the left eye with mud. He was wearing safety glasses. We washed his eye & transported him to ER for further treatment.

Rock fell from roof measuring 48"" x 27"" x 4 1/2"" thick striking member on hard hat driving it down on nose. It then struck member on right foot.

Employee was carrying O2 bottles to the stage loader when he slipped and fell. The employee was walking in xcut 1x2 last open and he felt pain in his lower back.

Getting off truck at end of shift foot slipped off of bottom step fell onto ground and hit left arm on ground

Employee was knocking a hole in a stopping and turned the sledge hammer on its end. He hit the stopping with the head of the hammer, catching his finger between the hammer and the stopping. *Lost time reportable injury 12/19/06.

Person was operating shuttle car; the car dropped into a hole in roadway causing operator to strike his head against canopy. Person went to doctor and released for work on 9/22/06. He worked 9/26 & 9/27. On 9/28 he reported off work, returned to doctor and is now off work pending diagnosis and treatment.

He was attempting to put a wheel back on the mantrip by pushing on the axle with a bar. When doing so the wheel turned causing him to twist his left knee. On 10-7-06 he had a MRI. On 10-20-06 he learn that he would need surgery to repair 2 tears in his left knee.

Replacing vee belts on crusher drive. Caught tip of little finger on right hand between vee belt and drive sheave.

EMPLOYEE WAS PUTTING UP AN OUTSIDE BOLT WHEN A PIECE OF ROCK FELL STRIKING EMPLOYEE ON THE SHOULDER.

Employee building up seal area on cone and using a grinder. A piece of steel flew into his right eye.

Employee was climbing ladder when dust fell behind safety glasses and got in his left eye. Employee left plant thinking his eye normal and awoke the next morning with redness and irratation. He called in said he was going to stop at eye clinic and have it checked. He returned to work without restrictions on 9/23/06.

Miner operator had finished loading shuttle car. After the buggy went around the turn the miner operator stood up & walked backwards. While doing this, he fell backwards stumbling over a lump of coal. He was given first aid by EMTs and transported to surface. He was then taken by ambulance to Ruby Mtn. Hospital.

He was installing a bolt, the bolt head broke off and the plate and pan struck him on his left forearm and cut a 3"" cut.

Employee was hooking up mantrip to cars. The mantrip operator trammed forward and caught employee's middle finger between the couplings.

While loading supplies onto bolter, rob roll struck left side, injuring neck and back area.

Employee was cleaning out the dust box on #1 bolter & pulled on filter throwing dust in his eye.

While lifting a concrete barrier with crane the injured reported that he may have hit the wrong control then over-corrected resulting in his right hand being crushed.

Haning Hi-line and stained lower back. Started missing work on 9/25/06.

Subject was operating stiffleg crane. Started to open window, hand slipped and hit window breaking the window cutting hand.

Putting the window down in the crane. Bees flew toward employee. He is allergic to bees so he jumped off the crane and hurt his left knee.

An A-40 Volvo off-road truck rolled over on a slope while hauling clay for the construction of the soil liner in area 4.

Dozer opr. was standing near dozer while welder was welding on blade. He called me approx. 5:50 am the next morning to say welding glare had burned his eyes.

Employee was pulling on a piece of conveyor belt loading it to take underground when he slipped and fell. As he fell he twisted his right leg resulting in a fracture to his lower leg.

Employee was pushing material up on the ballast stockpile. The rear suspension support of the operator's seat broke. This caused employee's back to hurt. He reported the incident and was taken to the clinic for medical evaluation.

The employee caught his toe on steps going down from the Bradley mill and twisted his ankle.

EE was placing a shaft in a bench vise. The shaft slipped and caught the EE's left hand between the shaft and the vise. Result was a fractured finger in his left hand. No lost workdays, restricted duty only.

Victim was securing equipment from loading dock (warehouse) to truck, when he lost his balance and fell approx 4' to the pavement. Left ankle sprain.

Setting on tool box rock fell out between bolts and hit him in back between shoulder baldes.

Cleaning out roadway to move S/C up entry. Picked up a 2' piece of 8"" I beam (2 beams welded together). When he picked it up he felt his back pop 3 times and got weak in the legs. Felt pain in his lower back. He finished the shift and then went to the doctor.

EE was moving (breaking apart) CM conveyor, when chain came apart. It sprung back, hitting EE on the knee.

EE stated he was assisting the maintenance man on changing out a broken head torque shaft. He said he felt a pain in his right knee when he climbed back off the miner. EE stepped down exactly 48"" from the top of the miner to the mine floor.

Employee claims to have injured his back when he jumped off of a man-up & landed on his feet & then fell.He stated that he saw the top move.

Emplyee was tramming the bolter in #2 entry to install a row of bolts when a piece of rock fell from between pins, hitting him in the back of the head pushing his face into the pinner tray. He received a broken nose.

Employee was knocked off his feet by cable.

Employee was at the tail drive preparing to build cribs where a high cavity had formed. He had started to set cribs. A piece of sandstone broke off above them striking the tip of shield 173, causing it to turn striking him in the back. On 10-13-06 he was removed from work by Dr. due to pain. He also was informed that he had fractured his #10 rib.

The operator on the 53-R drill was preparing to make a move to another drill. The operator got off the drill and began moving the cable with a cable hook, while pulling the cable the operator slipped and fell to the ground hitting her head on the ground.

Employee was operating the roof bolter when his left knee started hurting. He continued to work on regular duty without lost time until 1/17/07 when he underwent surgery for a meniscus tear.

Injured was changing batteries on scoop. He placed hand on edge of battery tray when lid fell striking his left hand.

Employee was moving miner cable out of roadway and strained his right side.

Employee states as he was helping hang a ventilation curtain, several pieces of rib broke loose. It fell & struck him on the back of the head, shoulders & upper back. The force caused the employee to fall backwards & hit his right hip against a metal fan frame assembly.

Conveyor roller caught hammer, pulling hand into a roller.

Member had just finished delivering a pallet of rock dust to the section with forklift; as he was tramming up the #6 entry toward the supply area, he trammed up on a small ledge causing him to jam his neck into the top. X-rays were negative.

Employee was cleaning off grizzly bars on the screen plant. He was on the platform leading into the loader cab. He was behind the platform railing pulling plastic from the grizzly bars when he slipped and fell between the loader and the screen plant. His left foot contacted the loader step fracturing his left ankle.

Injured was cleaning an area around a belt junction when his shovel got caught between a roller and the belt, twisting his wrist.

Co-worker stated that he and employee were helping do maintenance work on conveyor. Chain was fouled. He said they went to look if any miner bits were missing to see if that might have fouled chain. Co-worker was at bits, heard noise, looked and employee was laying on the ground with a piece of rock laying beside him.

Employee was in the process of lifting a steel rail bar from a supply trailer when he felt a pain in his back. The employee continued working until the pain worsened and had to see a physcian which instructed him to be away from work, First day missed was 12-18-06.

Employee was attempting to install roof bolt. He was trying to straighten bolt and guide the bolt to the drill head of bolter so bolter could assist in pushing bolt up into the drill hole when the drill head caught bolt bending the same and caught finger between roof bolt plate and the drill head.

Iron Worker was unloading steel beams off semi-tractor trailer, when load shifted causing worker to stumble and fall off of trailer (approx 5') landing on frozen ground. He landed on equipment tracks in mud that caused him to twist his ankle and fall on to his right shoulder. He is a ee of Pinnacle Construction a sub-contractor for Moltz Construction.

Injured was turning to walk away from roof bolter when he got his foot wedged between two rocks and twisted his right inside leg near knee.

Employee was turning around on the bumper of his service truck and he fell off the bumper onto a driveline.

Employee was riding in rock truck when rock truck slid down road & hit dozer on his side of the truck, causing injury to his Right Shoulder.

Employee states as he was pulling on cm cable helping to position it against the rib, he felt a strain in his lower back. He was referred for physical therapy.

Miner states that he was placing a guard over the drive belt that had fallen off. His glove came in contact with the belt and the pulley, pulling his thumb into the belt.

Rock dusting, main east, returned, felt pain in lower part of right leg. Diagnosed on 9/22/06. MRI report received 9/23/06.

Employee was taking banding outside to the dumpster when he waved the forklift operator down so he could get employee another bundle. As forklift operator was leaving, the banding got caught u nder the tire running over employee's left foot. On Going.

EE was riding in mantrip when a locomotive and the mantrip inadvertantly collided on track at #4 head drive. The key steel fell out of front drive unit on a hill and caused the mantrip to slide back down the hill striking the following locomotive. EE hurt his right knee and ankle.

While attempting to locate the heater valves in the engine compartment of a blade, the employee fell approximately 3 and 1/2 feet to the ground. The employee felt sharp pain in his right shoulder and both hips. He was unable to return to his next scheduled shift.

Employee was dismounting his dozer when he stepped on dust that had become slippery due to the steam ginny spraying out his radiator. The employee fell, landing on his left foot.

He was setting prop setters when top board fell pulling left arm behind head, straining shoulder. ** started missing work on 12/15/06**

While working underground lifting timbers and twisting at the same time caused some lower back pain and an injury to the rihgt hip area.

Employee reports while being loaded by shovel, shovel hit haul truck, jarring employee's back

A D10T dozer caught on fire and the employee was assisting in fighting the fire when he inhaled some fire extinguisher dust and smoke from the fire. The employee became sick and was taken to the E.R.

Employee put his hand around the base of the drill steel caught his glove between the steel and the chuck. When he started his rotation it twisted his finger resulting in a dislocation of his left ring finger.

Picking up the end of a welding trailer and felt a pain in his right groin. An attempt was made to report this on-line 9-21-06 but it did not go thru.

Employee fell between stack of pallets & #2 super sack machine.

Employee was assisting in removal of conveyor frame from old secondary surge tunnel. After support legs were cut loose, the frame moved, striking the employee's hip, causing a dislocation.

A contractor entered the bag house hopper for maintenance on a deflector plate. He entered the hopper without a lifeline or locking out the systems. While inside a large amount of hot lime dust was knocked loose from above, engulfing him. He got out before completely engulfed with several injuries, including a fractured hand, twisted wrist, bruised knee, ankle and lung damage.

Injured man was using a rope to drag timbers over a rock pile when he slipped and fell, twisting his right knee.

Injured did not feel any pain until the next morning in his lower back. He went to the doctor and had xrays and it showed he had inflammation in his lower back. He also went to a chiropractor who said he had 2 bones ""out of wack"".

Member was in the process of advancing shields, had hooked up 1"" pull chain to drag shield forward, when one of the hooks bent, causing chain to ties and strike him in the right arm and side. He received a compound fracture which required surgery to correct.

Employee was unloading some hose out of a pickup when he slipped on the tail gate. He received a cut on his right thumb and it later got infected, so he was sent off site and became an LTA.

Cutting a rag with belt knife, knife slipped and cut thumb.

Employee reported that he felt pain in his left knee cleaning out his operators compartment with his foot. Employee was diagnosed with a meniscus tear to left knee he started to miss time on 12/18/06.

Employee was assisting washing off cathodes in the tankhouse.

While Employee was moving a pump, the suction hose hung in rocks under pressure. The hose flipped back, hitting employee in the back & back of the Head.

Rock fell off miner boom and landed on employee's foot.

Injured empliyee was attempting to park mantrip in a charging station. Left front tire struck rib line, while attempting to back up from rib-steering wheel turned abruptly in oposite direction striking employee on ungloved hand.

Running away from roof fall, employee slipped and fell straining his groin.

The injured was working on #3 SC in the #6 entry, when a rock 1'x8""x4"" fell from the top, sliding down the right shoulder and right side of body. Bruised the right index finger and striking the right leg. No injuries other than the bruised finger and sore right leg were found.

Employee had just finished drilling hole and was walking along walkway at top of quarry. He slipped and fell into a void in the rock. Employee fell head first, hitting a rock, knocking him unconsious for 10-15 min. He has serious cut between eyes, possible fracture in nose, swollen right eye and bash in head.

Report accident 9/28/06-9/14/06 setting a post & post fall-he reach for it. Felt pain his rt groin-continue work-He was off work due to a hernia operation. The operation was 1/23/07 after confirmed on 3/9/07, that he did have hernia

Employee was carrying a cone pump down the steps and his right knee ""popped out"".

Phillips Electrical Technologies EE was drilling a hole to relocate a ground bar in an electrical panel. EE received an arc flash on his face, neck and arms. He was transported to Grady Hospital in Atlanta, GA for treatment of second degree burns and is expected to be released on 9/29/06.

Had quit dredging 1 hour before accident. Was taking old cable off of swing winch to replace with new. He was walking on ladder pulling cable when he slipped. He caught himself under his arm on handrail.

EE was operating loader cleaning bottom of Quarry face. Rock fell from face, striking bucket of loader, jarring operator in loader. Top of face is approx 30' high. EE continued to perform same job duties on scheduled shifts until 9/22/06, at which time medical professional personnel advised EE not to return to work until future examination scheduled for 9/25/06.

Was riding rail runner to section. Got cramp in leg. Raised up, too high. Hit mine roof. Knocked him back down in rail runner, he hit frame of rail runner with left hip.

Employee was helping to construct an overcast when he strained his lower back. He started missing 11-07-06.

Employee was climbing down off of the platform of a Cat 5130 Excavator attempting to put his foot on the first rung of the ladder when his footing slipped off the rung and the employee fell to the ground landing on his feet.

Employee was doing normal duties of lowering hose into hole. As he was doing this, he felt pain in his lower back.

Employee was on ladder trying to align sub to casing. Told co-worker to lower head down. When he did this, injured's foot slipped to pinch point between frame and drill head.

Was operating roof bolter. A rock 1 foot round and 1.5 inches thick fell hitting his finger and cutting it.

EE walking backward rock dusting-stepped in hole where floor cut unevenly. Hurt arm and shoulder, when he fell backwards hit scoop before landing on mine floor. Contusions to shoulder and forearm worked 14th-15th pain worse swollen not on shoulder wnt to dr. Hospital refer him to dr. on 20th. He is to do MRI on 9-21-06.

EE stooped down behind manlift to put object in box, when manlift, being operated by another EE, moved less than one foot in reverse. Struck rear left foot of injured EE, resulting in bone fracture. Manlift travel alarms were working at the time of the accident.

Employee was pulling on cable when employee slipped in mud and twisted right knee. (Employee did not start missing time until 10-19-06 due to surgery to right knee.)

The employee stated he was sitting in a mine jeep when a locomotive struck another vehicle which struck his in a chain reaction. The employee was diagnosed with degenerative changes to his left knee and left elbow.

Pulling wire, twisted wire around wrist.

Employee was lifting a roll of chain link fence, felt a strain in the groin area. Diagnosed as a hernia and repaired.

EMPLOYEE PUTTING ROCK DUST ON SCOOP WHEN ANOTHER EMPLOYEE DROPPED BUCKET ON EMPLOYEE'S LEFT FOOT.

Passing brick to bricklayers who were laying brick in kiln. Stock board gave way when another laborer stepped onto it dropping brick off which landed on left foot.

88 1/2 blk 3 west track haulage installing roof bolt aligning. Bolter mast Dropped catching r. little finger between roof bolt and roof plate-laceration of lower knuckle r. little finger 7 sutures.

Drill steel got hung in drill hole. Was pulling steel out of hole, with a lot of down force. Struck ring finger on left hand, between steel and drill head.

Lime burns to eye

Employee strained lower back while lifting a piece of hand rail.

Injured was helping clean coal off the #3 tailpiece when he had his finger hit by his co-worker's hammer. Injured went to push a piece of coal off tail piece at the same time his co-worker hit the same piece of coal with his hammer.

Employee was dismounting his dozer, when his foot slipped and he fell into the lower handrail striking his left side of rib cage.

Employee was backing roof bolter out of section three left. He had his left arm out of deck, while holding cable out of way. His left arm got caught between the roof bolter and rib.

EE was dragging a base-lift up the face when he felt a pop in his wrist. EE did not go to the doctor until 10/5/2006 ( 25 days later).

EE was carrying some hose and tripped, landing on an old pump pedestal injuring his right knee.

On September 12, the employee was cutting off old patches in a chute. He then was chipping the hot slag when a small piece struck his face below his safety glasses. The injury did not become recordable until 10/12/06 when he went to the doctor for the injury. His eye became infected as a result of the minor burn.

Ee was exiting doorway to t-buck while forklift was making it's way or ramp to doorway. The honking signal was not given in time and the ee was hit by the forks in both knees. one knee (R) is just bruised, (L) badly bruise. xrays show no briken bones althought mri is pending to determine damage to cartilage.

Injured alleged he was setting rock off of rock crusher and experienced pain in his left shoulder. Worked 9/13/06 & was off 9/14 & 9/15.

Pulled a muscle in his back (left side) while lifting CM cable & water hose.

While returning to operations at shift change, the van hit a pot hole and employee hit her head on the roof of the van, causing neck and lower back pain. He was wearing seatbelt. She was taken to the ER, referred to her personal physician, and has been taken off work until completion of physical therapy. ONGOING

Person was putting roof bolts up and got glue in right eye at #4 heading.

Injured struck his head against a crib block that was set over a timber for roof support. The area was about 60 inches high. Injured began missing work as a result of this injury on 9/18/06

Bolting top stepped on a slick rock and twisted ankle.

EE was stepping through mandoor, slipped and fell, injuring lower back and hip. Began missing work on 9/14/06.

DID NOT START LOSING TIME UNTIL 1/10/07. While tramming feeder, stumbled and twisted left ankle. Surgery required on 1/10/07.

Employee was lifting slate rock from the rock crusher and hurt his back. He first sought medical attention on 9-26-06. Did not miss work until November 1, 2006.

The man was moving belt structure when he slipped on wet bottom and felt a pain in his left groin area.

Was welding he got flash burns.

Victim was tapping in a miner bit insert into the bit lug on the miner, when an object (likely a metal fragment), struck him in the right lower eye lid, then entering the eye. *Note: Victim was wearing eye protection*

The employee tripped on a roof bolt header board causing him to fall onto left knee. Employee fractured knee cap requiring surgery to repair.

Rib rolled off striking employee across right shoulder and back.

Removing a bolt with a crescent wrench. Wrench slipped off the nut a struck EE in the nose. He had just had surgery for a dievated septum and was healing when he hit his nose with wrench. He had to have his nose re-set again.

Employee claims he was walking across hood of rock truck to clean right side mirror. Because of wet weather the hood was wet and slick. The employee slipped and fell onto the hood. THE EMPLOYEE DID NOT REPORT THE UNTIL 09-25-2006, AND DID NOT BEGIN LOSING TIME UNTIL 10-05-2006.

EE had Helper Tighting the setting of the crusher. a small size stone intzaferred with the hypaolic ram used for turning the top of the crusher. He reacher through the safety cover to remove the stone between strokes. The cylomone came out while his fenders were there, crushing then between the ram and rep of top.

Employee was cleaning/lifting rock from 60 in under jaw belt and shoveling. He felt a pull in left waist area. This was reported to the supervisor but nothing needed to be done. Several months later it was found to be a hernia and surgery was performed causing him to miss work.

Was getting on top of drag-line to grease the wheel. Stepped into some grease and fell.

Employee was hydroblasting using a standard length shotgun in the 22-1 sump pump in the Rodmill area. The employee cut the top of his left foot with the water jet from the shotgun resulting in a 1 1/2 inch long cut approximately 1 1/2 inches deep. This injury resulted in a workers compensation claim and loss time.The length of lost time has yet to be determined.

He was moving a piece of rail. He took a bar to roll rail over off ties, rail didn't move. He felt pain in lower back.

Employee was stepping down from a belt cross over and twisted his knee causing him to fall. The fall hyper extended his knee. The initial doctor visit released the employee to work. On 9-26-06 he return to his doctor and has been taken off work. This is why there is a difference from date of injury and date report was submitted.

Employee was pulling on a rope bolt from the roof bolter when he strained his right elbow. He did not start missing work due to this injury until 10/6/06.

employee was cleaning brick out of fuller kiln, got lime dust in left eye

Employee was bolting a belly pan back up on dozer after changing a starter. The chain holding the belly pan up came loose allowing it to strike his arm.

Employee was cleaning scrubber on CM when rib material fell on left leg and foot. Employee continued shift and alleged that his ankle swelled and he could not wear his boot.

EMPLOYEE WAS MOVING BOLTER ACROSS SECTION WHEN HE HIT HIS BACK ON ROOF BOLT CAUSING HIM TO FALL, WHICH CAUSED HIM TO TWIST HIS KNEE.

He stepped on a motor base in the top of an old loadout to look for a hole in the chute and slipped and fell on his back. He fell one foot to the ground, bruising his back.

Employee was helping to belt up - while moving belt structure employee experienced pain in upper back.

The employee tossed miner cable out of the roadway and felt pain in his lower back.

EE WAS HANGING CABLE OVER FEEDER TRAVELWAY. APPROX. 15 MINUTES LATER EE COMPLAINED OF PAIN IN ABDOMEN. LATER INFORMED EE HAD A BUMP/LUMP IN GROIN AREA.

EE picked up box of glue, twisted back and felt sharp pain in his lower back. EE did not seek medical attention until 11/18/06 when Dr. took him off work. Workers comp did not determine unitl 12/13/06 that the injury he was being treated for was work related.

Miner was pulling on cable and squatted down to walk under a low beam. Miner slipped in mud and twisted right knee resulting in an acute knee strain. This report is being filed on 05-24-2007 due to an audit.

Employee was loading tires into a truck when he felt pain in his lower back.

Pulling miner cable across intersection and strained his back. First day off 09-14-06

He was using a pry bar to remove a top roller frame and felt pain in his right bicep. He was diagnosed with a partial bicep tear of right arm.

Employee was bolting up the drop board and grabbed the steel, caught his glove and fractured his finger.

Two employees were placing a backstop for a ramp to a feeder hopper. The backstop, 5' x 15' x 1/2"" steel plate, was lifed vertical to lean against feeder, the groundman pushed up on the steel plate by hand to over center when he did that the chain came unhooked from the plate which caused the steel plate to fall toward him pinning his leg against the ground.

EE felt pain in right arm /shoulder removing steel from chuck of bolter. MRI negative findings.

Member was dismounting a 777 D Rock Truck, when his knee popped.

Employee was opening a fuel fill cap on storage tank on truck, spring loaded lock on cap hit him in the face.

Employee was cutting in right hand x-cut when he for some reason advanced inby permanent roof support and was struck with a piece of falling roof.

Employee was getting off loader when he slipped on mud injuring his right shoulder and neck.

Injured was lifting 5 gallon buckets of oil from a flat car when he felt a pain in his chest.

Employee was taking lid from ball mill. When lid started to fall he tried to catch it resulting in injury.

Was lifting car tounge up to move into position to couple up. Pain middle back.

Employee was operating a diesel ram car, backed over material laying in roadway and was thrown into canopy of operator's deck.

A piece of drawrock fell out from between the roof bolts, striking employee on his head & left forearm.

Employee was loading drill into #8 man hole, injured and twisted right knee.

Employee pulled ladder down on equipment in order to climb up. Ladder did not go all the way down, when employee attempted to go up ladder, his foot slipped and caused his Left Knee to hit ladder.

Employee was walking across section when he lost his footing due to wet condition, causing him to twist his left knee. He continued to work until 9/12/06.

Employee was operating the Track hoe in the pit. He backed the machine over a 2' to 3' diameter rock. The resulting jar caused him to tear abdominal muscles. Unsure of the tear's severity until MRI is performed.

Employee bumped his elbow and felt pain. He noticed a small red bump and squeezed it and a piece of metal came out. He went to the hospital and was released. He returned due to pain and the doctor admitted him to the hospital. He remembered he had bumped it two weeks prior. It was not known if the infection was from a work related injury, but we are treating it as a work comp.

Employee claims he was getting off of the RSP Tower elevator on the 3rd Floor. When he closed the door, with a broom in his right hand, the broom caught in the door breaking his right thumb.

Grinding & welding & got metal in left eye.

Injured party was trying to pull start a gasoline pump. After the second time he pulled the start cord, it did not catch as it should have, causing the injured party to strike his left hand against a piece of equipment.

Ee was loading ridge rails in supply car when his left side ring finger was caught between 2 pieces of fridge rail causing accident.

Examiner was coming down an aircourse and a rock was in the middle, he went to kick it out of the way and something popped in his right calf of his leg. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equip not a factor, miner proficiency a factor, mining equip/systems not a factor

Employee was walking on the ground floor of the moly plant when he slipped and fell on the wet moly fracturing his right shoulder.

Employee was walking and tripped on uneven ground, fell landing on a rock hurting his left knee.

Ee was working on brattices. He had removed part of one. Unknown to him, a block had stuck to the top until he got under it and it fell, hitting him in the back.

Employee stated that he was using a torch cutting an Axle Pin out from underneath a Shuttle car. A piece of hot slag popped off the pin and went into his ear. He has not missed any work. Started missing 11/27/2006 with surgery to repair a hole. This was not turned in to me until 9/27th.

Riding 3-wheel mantrip, he hit his head on mine roof.

Employee was talking to another employee who was operating a mini trac. A third employee was loading crib blocks on a golf cart when he accidently placed one on the tram pedal of the golf cart. This caused the golf cart to start tramming and hit employee on the legs pinning them against the mini trac.

Replacing screen on Siene Bend-while replacing screen strained left lower back-worked until 9/18/07

Employee was operating haul trk when he hit an obsticle in road, jarring the haul truck and causing the pulled muscle in lower back.

Miner was stepping off of a trailer and stepped onto a chunk of coal spraining his ankle.

While a mechanic was performing the task of cutting safety glass, methanol vapors ignited burning the mechanic's left arm and hand. The mechanic incurred 2nd degree burns to his left hand and forearm and 3rd degree burns to the finger tips of his left hand. The mechanic was transported to a medical facility and did not return to his next scheduled shift.
