Cutting coal on the longwall face at Nor 76 shield when an ignition occurred at the top of the headgate drum.

An ignition of methane occurred in the fresh water pump house causing damage to the structure. Inadvertently, methane was liberated from the well, accumulated in the structure and when the pump turned on the ignition occurred.

A roof fall occurred in the 15 North Panel #10 entry in the intersection of cross-cut #23 and extending through the pillar line toward #9 entry measuring 50ft. x 20ft. x 6ft.- 9ft. The area was supported with 5ft. fully grouted bolts and 9ft. mega bolts. An undetected slip was determined to be the cause of the fall. The area was mined in August 2006.

While operating the hoe, a newly installed hose blew an end & sprayed hot oil over the engine & turbos igniting a fire that quickly got out of control. Fire extinguishers had no effect. 1st fire department called deployed to the wrong place & a 2nd fire dept. had to be called. The fire depts did not fight fire, but made sure it stayed contained. Burned out @ 2:00pm no injuries

An unintentional roof fall occurred at the #14 brk, 3 West #2 belt in the #3 entry. Size of fall was estimated to be (5'H x 18'-20'W x 70'L). Fall will not be cleaned up. No injuries occurred, no equipment caught, ventilation was not interrupted. Area had been bolted with 6' torque tension bolts.

Roof fall between spad #133 and #134 in #1 entry on 1 north mains. Fall measured 65 ft. long, 19 ft. wide by 8 ft. thick, approximately 9,200 ft. outby the section. Rotten top and water present was determined as the cause.

Found roof fall at 1:00 p.m., 1 break out by spad #3942 in #8 entry on 2 left mains. Fall measured 35 ft. long, 19 ft. wide by 4 ft. thick, approximately 1,800 ft. outby the section. Rotten top was determined as the cause.

A roof fall occurred on the 2 Left, 4 West panel at crosscut 16 in entry #2. The cause of the fall was weak laminated shale commonly referred to a stack rock. The fall was located in the weekly examiners normal route of travel. His route of travel was rerouted around the fall.

Elevator would not close properly, because of the retiring cam. Men were brought in and out of the mine by the slope entrance. The elevator was repaired and running by the end of the day shift at 4pm.

A roof fall occurred in the main east from crosscut 6 to 7 in #6 entry. The fall measures approximately 70 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts and truss bolts.

A inactive hole (gas or oil) was intersected on the midnight LW shift at 21 Shield of the 12 East longwall section. No accumulation of methane was found but was liberating 2.6% over the hole. The hole was opened up by using non-sparking tools and plugged. THE OPERATOR IS FILING THIS REPORT IN PROTEST AS MSHA IS SAYING THERE WAS AN INUNDATION OF GAS. NO ACCUMULATION WAS FOUND.

A roof fall was discovered in the 1 west #5 panel, crosscut 7 to 8, #2 entry. The fall measures approximately 30 feet long, 18 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts and 10 foot point anchor bolts.

A roof fall was found at 12:15 pm on 9/25/2006 by examiner doing a pre-shift. Fall was in four way intersection at 48 cross cut entry one of 5RMS. Area was mined March 2003 and had been supported with 8' full resin bolts. No injury or property damage resulted. There are no current plans to clean up fall.

On 9/24/06 an unintentional roof fall occurred on the 2nd 42 South Belt #5 entry at #60 crosscut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 25'l x 18'w x 14' H and the roof fall will be cleaned up and resupported.

9/23 & 9/24 with reported 10"" of rain (guage at mine went to 6"") overwhelmed the sump at the drift mine pit and the 8"" electric pump and power went off and the 8"" diesel backup's clutch went out and water ran into the mine inundating it for the 300' it had advanced. Water roofed on left side 4th & 5th xcuts (#1 entry) and water 3000' at 4th xcut in #7 entry and face.

A roof fall occurred on the 2 Left, 4 West panel in entry #2 between crosscuts 43 and 44. The cause of the fall was weak laminated shale (stack rock). The fall was located in some abandoned coal hauler stations adjacent to the section roadway.

A supervisor walked out of the building and saw in the distance what he initially thought was smoke or a mist which was close to ground level. It was several hundred feet outback of the building. Upon closer investigation, it was determined to be gas spewing up through the ground.

1/2 Break outby No. 3 Belt Tailpiece, wet rotten conditions caused a section of the roof to fall. The fall is 25 ft. long by 18 ft. wide by 6 to 12 ft. thick approximately 9,000 ft. from the active section.

A roof fall was discovered 1 break inby spad 972 in #3 entry. The estimated size of the roof fall measured 6' high, 18' wide and 20' long. No injuries were reported as a result of this roof fall.

Oldenburg Scaler in service since Jan. 05 caught fire while scaling in S-27 1600' level. The fire was probably caused by a hydraulic leak ingnited by normal operating temps of the engine. The scaler was a total loss.

On 9/21/06 a blast pattern was detonated in the main pit and seconds later the south high wall just above the blast sloughed down. Appoximately 500,000 tons.

At approximately 12:55 on the North Left section, MMU 003-0, 18 block, x-cut #3 to #2 there was an alledged flash generated from cutting sandstone. The intial investigation by MSHA and the state determined negative findings.

An unintentional fall of roof was discovered on a weekly exam in east-west mains, 1st panel, right break #47. The fall was approx 20 x 20 x 8.

Examiner found roof fall at the 1360' in x-cut between #5 & #4 entry of the 2nd east entry along the 1A belt. Fall is 35 - 40' long, 17 - 18' wide, 7 - 8' high. Primary roof support was 5' double lock. Fall has been timbered on both ends. Additional support has been installed on roadway. Fall will not be cleaned up.

Power went out causing hoist to go down at 8:40 A.M. Problem was with electric company (Amerun UE). Hoist was back in service at 11:30 A.M. MSHA was notified of power outage and when power was restored.

An unintentional roof fall occurred at the #32 Crosscut/Flannigan belt. The fall was approximately 20 feet long, 17 feet wide and 15 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up.

Approximately 30 minutes after cutting the #3 right x-cut thru into the #4 entry (@ #25 xc at station spad 1:+91) creating a three (3) way intersection, the intersection of #4 entry fell in. The intersection was bolted with six (6) ft fully grouted roof bolts and less than 4 ft contours. No one was injured, no equipment was damaged.

During the semi-annual NDT testing, the Koepe mancage hoist ropes showed increased wear, enough to warrant changing. The mine was evacuated and ropes will be changed.

A roof of mine roof occurred on the 005-0 section, 3 right panel, #4 entry at #9 crosscut. The fall was approximately 19' wide, 35' long and 6' thick. The fall occurred during retreat mining, catching the Joy continuous miner and 2 MRS's. The material was removed and the equipment removed safely.

A roof fall occurred on the Main North Track at 43 x cut, 04 Entry, 40 + 70 feet (old 4 West Track Spur). Fall was 30' long, 20' wide, 5' high. Area was in June, 1989. Fall occurred due to deterioration of the immediate roof around the existing roof bolts. Area was supported with 8' roof bolts, resin anchored 2' expander shells. Plank, hedder block & 12' cable bolts were used.

An unintentional roof fall was discovered on the #1 section, 1 break inby spad station # 5029, from #7 entry thru #9 entry in the last open X-cut. The fall measured 90' long x 17' wide x 11' high. This miner section has been withdrawn and ceased mining.

A roof fall was found at x-cut 76 entry 4,5R main south. Fall was in four way intersection that had been mined October 21, 2004. Area was bolted with 6 & 8 foot full resin pins. No injury or property damage resulted. There are no plans to clean up fall.

A rock rall occured in the 1 main south, #6 entry, in part of the pillar line between crosscut #5 and #6. The fall measured approximately 20'x19'x6'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.

A roof fall occurred in the main east, crosscut #37, between the belt and travelway. The fall was approx. 18' long, 20' wide and 6' tall. The area was bolted with 6' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.

The hoist was being prepared to pull a continuous miner out of the slope from the under ground.The brake car was in the side yard switch chained off. While pulling the hoist to double sheave the rope, slack developed on the hoist drum causing the rope to tanle and due to the tangled rope management at that time decided to change out hoist rope. The hoist was down from 9:14-5:02 pm.

A roof fall was found on 9/16/2006 at x-cut 160 entry 1 main south. Fall was in four way intersection that had been mined December 11, 1996. Area was bolted with 6 and 8 foot full resin pins. No injury or property damage resulted. There are no plans to clean up fall.

A fire was observed behind the cab of the bulk emullsion truck. The truck was fully involved within minutes. The blast pattern was evacuated and approximately 70 minutes after the fire started an explosion occurred. The bulk emullsion truck was a total loss.

The man hoist would not operate, trouble shooting found several fuses blown. There was a thunderstorm in the area, possibly a current spike blew the fuses. The hoist was out of service for 35 minutes. Two other man hoists were operable.

A roof fall occurred 10'-15' inby survey station #6023 in the No. 3 entry on the 003-0 section. The fall measured approx 40' long x 20' w x 8' thick. The area had been bolted with 48"" resin bolts & supplemented with 12' cable bolts. The fall occurred in an area of known bad roof & began working on the 3rd shift. The area was dangered off. The fall occurred at approx 6:35 am.

A roof fall occured in the 4 East panel, #8 entry, intersection of crosscut #58. The fall measured approximately 19'x19'x7'-9'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.

Employee was in the process of lifting a steel rail bar from a supply trailer when he felt a pain in his back. The employee continued working until the pain worsened and had to see a physcian which instructed him to be away from work, First day missed was 12-18-06.

Employee was attempting to install roof bolt. He was trying to straighten bolt and guide the bolt to the drill head of bolter so bolter could assist in pushing bolt up into the drill hole when the drill head caught bolt bending the same and caught finger between roof bolt plate and the drill head.

Iron Worker was unloading steel beams off semi-tractor trailer, when load shifted causing worker to stumble and fall off of trailer (approx 5') landing on frozen ground. He landed on equipment tracks in mud that caused him to twist his ankle and fall on to his right shoulder. He is a ee of Pinnacle Construction a sub-contractor for Moltz Construction.

A contractor entered the bag house hopper for maintenance on a deflector plate. He entered the hopper without a lifeline or locking out the systems. While inside a large amount of hot lime dust was knocked loose from above, engulfing him. He got out before completely engulfed with several injuries, including a fractured hand, twisted wrist, bruised knee, ankle and lung damage.

Phillips Electrical Technologies EE was drilling a hole to relocate a ground bar in an electrical panel. EE received an arc flash on his face, neck and arms. He was transported to Grady Hospital in Atlanta, GA for treatment of second degree burns and is expected to be released on 9/29/06.

Had quit dredging 1 hour before accident. Was taking old cable off of swing winch to replace with new. He was walking on ladder pulling cable when he slipped. He caught himself under his arm on handrail.

EE was operating loader cleaning bottom of Quarry face. Rock fell from face, striking bucket of loader, jarring operator in loader. Top of face is approx 30' high. EE continued to perform same job duties on scheduled shifts until 9/22/06, at which time medical professional personnel advised EE not to return to work until future examination scheduled for 9/25/06.

EE stooped down behind manlift to put object in box, when manlift, being operated by another EE, moved less than one foot in reverse. Struck rear left foot of injured EE, resulting in bone fracture. Manlift travel alarms were working at the time of the accident.

On September 12, the employee was cutting off old patches in a chute. He then was chipping the hot slag when a small piece struck his face below his safety glasses. The injury did not become recordable until 10/12/06 when he went to the doctor for the injury. His eye became infected as a result of the minor burn.

Ee was exiting doorway to t-buck while forklift was making it's way or ramp to doorway. The honking signal was not given in time and the ee was hit by the forks in both knees. one knee (R) is just bruised, (L) badly bruise. xrays show no briken bones althought mri is pending to determine damage to cartilage.

He reacher through the safety cover to remove the stone between strokes. The cylomone came out while his fenders were there, crushing then between the ram and rep of top.

Employee was hydroblasting using a standard length shotgun in the 22-1 sump pump in the Rodmill area. The employee cut the top of his left foot with the water jet from the shotgun resulting in a 1 1/2 inch long cut approximately 1 1/2 inches deep. This injury resulted in a workers compensation claim and loss time.The length of lost time has yet to be determined.

Employee bumped his elbow and felt pain. He noticed a small red bump and squeezed it and a piece of metal came out. He went to the hospital and was released. He returned due to pain and the doctor admitted him to the hospital. He remembered he had bumped it two weeks prior. It was not known if the infection was from a work related injury, but we are treating it as a work comp.

Employee was talking to another employee who was operating a mini trac. A third employee was loading crib blocks on a golf cart when he accidently placed one on the tram pedal of the golf cart. While a mechanic was performing the task of cutting safety glass, methanol vapors ignited burning the mechanic's left arm and hand. The mechanic incurred 2nd degree burns to his left hand and forearm and 3rd degree burns to the finger tips of his left hand. The mechanic was transported to a medical facility and did not return to his next scheduled shift.
