Suspected fuel leak caused a fire on a caterpillar 345C Excavator. Machine is expected to be a complete loss.

A roof fall occurred in the main north return between x-cuts #146 and #147 in #1 entry. The fall measured 20' long x 12' wide x 6' high.
The crabapple hoist car (break car) was taken out of service for an extended period to make normal repairs to the car found during inspection.
Hoistman moving cagers from 3700 pocket to 3700 Station to transfer more muck. Stopped at upper pocket the #3 hoist stalled out and would not reset. Brake limit switch had to be readjusted.
At 6:10 a.m. it was reported the hoist was not working. MSHA was notified by phone at 6:15 a.m. After trouble shooing it was determined that the DHSS (Electronic Lilly switch) was not in the run mode, and a magnetic switch had moved causing the error. After corrections were made and hoist tested several times hoist was put back in service at app. 9:30 a.m.
Roof fall above the anchor zone in the intersection of Mine 5 LOB seam XC-204 in the #2 entry.
A hot area in the exposed coal face was fanned by winds and produced flames which were extinguished by a dozer operator but not within the half hour after discovery. No miners were ever in danger at any time.
Caterpillar 345 scaler was operating in 21N 43W North mine. Fire started behind cab. Operator was unable to control fire and abandoned machine. Notified other miners and supervisor and evacuated mine.
A roof fall was found by a pumper. This fall was not in an escapeway or travelway. It was in a return crosscut. There was no change in ventilation.
We lowered the maintenance cage for the monthly visual inspection and the cage hung up in the shaft. We lowered a remote camera into the shaft to get a visual of any bulges that were created by recent rains and saturated ground conditions. All bulges were corrected and the shaft was returned for use.
On May 24, 2019 at 7:00 pm Grader 4239 was reported catching fire in the NHW Ramp to TROC. the equipment was immediately stopped, shut down, the fire suppression deployed. It was not confirmed if the equipment was fully extinguished.
A fall of roof occurred on the 2nd South submain in entry # 10, xc # 8. The fall measured approximately 18 feet wide x 18 feet long and the thickness is unknown.
Employee became trapped on Camp Run Elevator when a rope gripper fault occurred. Maintenance was contacted to reset the fault. No injury to any employee & no damage was found to the elevator. Elevator returned to service at 7:56 pm & the trapped employee was off the elevator at approx. 8:09 pm. An Elevator Service company performed follow-up inspection & no problems were found.
There was an unplanned roof fall at #3A Right Return Spad 3+90 #7 Entry.
The elevator at BMX portal was out of service due to the bottom door sensors malfunctioning. The sensors and the door track were cleaned and the elevator returned to service.
Employee was trapped in the Step 3 freight elevator at the concentrator around 9:45pm. MSHA was notified at 10:11pm. The elevator company responded and the employee was freed at approximately 11:20pm. The elevator was put back in service.
Power was lost to the main hoist last night (5/22) at approximately 10:15PM. Employee called the hotline to report the hoist being down. Power was restored at approximately 1:20AM (5/23) and employee contacted the hotline again at that time. Power loss was due to a line fuse blown on KU's end.
The Archer portal elevator was having issues with the programming of primary and secondary slow down sequence switch. The switch was replaced.
The Hoist car was inoperable for an extended period of time. There was a malfunction in the cars braking sensors that caused the breaks to lock up. The sensor wires became damaged and the manufacture was contacted and made the necessary repairs
Miners went to their work area on 2000 East Footwall Lateral & seen the back had started to fall out at the working face. They called their supervisor who then went to investigate & seen where one 6 foot swellex bolt had fallen out from the previous bolt up making this a reportable incident. EE made a call to the superintendent who made the MSHA hotline call at approximate 3:00 pm
Rock Fall was found at 12:59PM. Approximately 100-150 feet underground, near Fan Entry, First Break in Return. Rock Fall blocked return where you couldn't travel return in its entirety.
An unplanned roof fall occurred in the intersection of cross-cut #12, entry #4, of the 3B10 panel at an unknown time on May 18, 2019. The roof fall is estimated to be 20 feet high. Entries approaching the fall measure approximately 18.5 feet wide.
The Archer portal elevator was inadvertently put into inspection mode.
At approximately 11:55am a loud noise occurred that alerted the office personnel. Upon investigation it was discovered the top of the surface bulk hydraulic oil tank had blown off. Investigation revealed the hydraulic oil level was lower than the electric heating element warming vapors to combustible levels. No one was in the area at the time of the event.
An unplanned fire occurred on the company number 4 water car.
A roof fall was found near spad #2552. No employees were injured, no equipment involved, ventilation was not affected.Additional roof supports in the form of timbers were set to prevent reoccurrence. The fall was 50' in length, 10' in height, 19' in width. This fall will not be cleaned up. The lifeline and primary escapeway was re-routed.
Dry spring grass was ignited by sparking from carbon build up in locomotive exhaust stack. Damage was limited to grass on property. No equipment or other asset damage. No injuries.
Two process utility men were riding down in the preheater elevator to the first floor when it came to a complete stop between 2nd and 3rd floor. A technician for the elevator company arrived and was able to get the employees out at 10:50 AM. The 9th-floor door to the elevator shaft had a mechanical bind that engaged safety interlocks to hard stop the elevator.
#1 Bore Miner mined into a small void of gas and water. With immediate ventilation changes, gasses were dispelled and the mining process proceeded within 15 minutes. The MSHA hotline was notified due to Solvay Chemicals receiving a citation (9348706) the prior week for not immediately reporting the same issue as a rock outburst.
This is an older area of the mine that was intake for 10+ years and is now return. The intersection that fell is at the bottom of a dip, and appears to have been a large localized horseback. There should be no effect to ventilation either short or long term, as there are multiple additional return entries. No one was injured in the accident.
Operator was loading trucks and noticed that high idle quit working then am/fm radio quit then CB quit, when they looked back they saw smoke coming from the engine compartment and tried to shut the machine off. The wire going to the engine shut off had burnt into also so they could not shut it down. Operator got off the machine and walked away
An Unplanned and Reportable Roof Fall occurred at #26 Block, #8 entry outby the section tailpiece on the D-Mains Working Section (MMU 001-0 & 005-0). There was no injuries, no equipment damage and the ventilation system was not disrupted. The occurrence was immediately reported.
Safety Dept. on-call person received a call @ 1:50 pm reporting that a manager was trapped in the Conc. Step 3 elevator. MEI was contacted & they stated they would send someone out to repair the elevator. Safety Dept. notified MSHA @ 2pm to report the entrapment and again at 4:30 when employee was rescued.
A roof fall occurred inby the feeder on 5/11/2019. Break 24 in #1 entry to brk 26 in the #1 entry. Then from brk 26 in 0 entry in brk 29 in 0 entry, all in left return. Area has been cribbed off in all approaches to fall and cable bolted with 12' cable bolts.
Operate Mucker 2-4 (250M) heading up incline to get another load of muck, looked back saw flames coming from end of mucker. Articulated mucker into rib and hit fire suppression system
A roof fall occurred outby 001 section in the #1 entry at x-cut 61. The fall was approximately 16 feet long x 20 feet wide x 6 feet in height. The fall did not have any effect on persons, equipment nor ventilation. An action plan was submitted and the area was re-supported.
#34 HOIST WENT DOWN AT 7:20 AM. MSHA WAS CALLED AT 7:35 AM. SURFACE MAINTENANCE REPLACED 3 BUSHING IN GEAR BOX THAT ROTATES MECHANICALLY AND TELLS THE HOSITMAN THE CAGE LOCATION. HOIST WAS RUNNING, INSPECTED, AND PLACED BACK INTO SERVICE AT 08:00 PM. MSHA WAS CALLED AT 08:12 PM REPORTING THE HOIST IN OPERATION.
The roof strata in the immediate area of the fall appears to be on localized stack rock that has deteriorated over time. THIS IS A NON-INJURY ROOF FALL
The Crabapple hoist was taken out of service for an extended period of time. An abnormality/slight bend in the hoist rope was found approx. 15 feet from the hoist car. It is unknown what caused the bend in the rope. The tension in the rope was released prior to finding the bad spot. No reports of issues on the slope bottom. An outside engineering company has been contacted
ROOF FALL - #1 to #2 entry big block roof fell out for 75 ft rib to rib, 20 feet wide, 6 to 8 feet high due to bolt spacing on 5 foot centers using 4 feet resign bolts and 8 feet cable bolts.
Roof fall found in the Johnson Fork Butt off's in the #3 entry, between 55 break to 56 break. Approximately 80 feet long, 19 feet wide and 8 feet thick. The fall does not impede travel or air movement. No injuries or entrapment. A location and map has been sent to Mount Hope Office.
A roof fall occurred above overcast at Main South Belt line at XC 27. The fall occurred when roof bolts pulled out of the strata in the roof. The material was 6"" thick x 10' long x 15'wide in size on top of the crushed overcast. Damage to the overcast disrupted main ventilation in the area. MS belt line was damaged for approx. 15' and no miner was injured in the event.
A roof fall was discovered in the plant mains at crosscut 22 between the #3 and #4 entry during a pre-shift examination. The roof fall is apx 25'ft long X 5'ft high X 15'wide. The area was dangered off and an action plan will be submitted to MSHA. The fall did not affect persons, ventilation, or equipment.
A roof fall was discovered in the bleeders of the old 17 headgate at break 92 to 70 ft outby 94break in the # 3 entry. Size of the fall was 18' wide x 7' thick x 140' long. The area has been dangered off.
A non-injury event occurred when the slope car mechanically malfunctioned. Production was halted and men withdrawn from the face to effect repairs. Attempts to correct the malfunction were unsuccessful all personnel were removed from the mine until repairs are made. Both primary and alternate escapeways were available for use. NO PERSONS WERE INJURED DURING THIS EVENT.
Miner was using a man lift basket to observe feed to cone crusher at a height of 28 feet. The miner was wearing fall protection harness and a retractable lanyard but was not secured/tied off to the man lift basket. The miner was ejected from the man lift basket to the surface below.
Crane fell over into quarry with operator inside cab. The victim was operating crane lifting an approximately 15 ton block of granite from the pit. The boom of the crane lowered causing the crane to tip over and fall 85 feet into the pit.
Employee caused a hernia due to lifting a piece of stone. EE went to the ER in Plains MT (Clark Fork Valley Hospital) for initial treatment. EE will be having surgery to repair the hernia.
Employee was center bolting, employee was retrieving the bolt wrench when they stumbled and contacted the lower ATRS control causing it to lower. This pinched their left index finger between the ATRS and the bolt wrench causing a laceration.
A mechanic was repositioning a crib block under a shuttle car when the shuttle car slid down inside the bucket of scoop approximately 1"" causing the car to pinch the employees thumb on the crib block. The employee lost approximately 1/2 inch of the tip of right thumb due to the accident.
An employee had been building cribs under the feeder and as EE was trying to keep balance, EE's right index finger got between two pieces of metal near the jack area smashing the finger. The employee was brought outside and taken to the hospital where it was determined EE would lose part of the finger.
Injured employee was entering the Prep. Plant Control Room. Opened door to room & stuck EE's right hand back to catch the door as it was closing behind EE. EE inadvertently stuck EE's right little finger in the hinged side of the door as it closed, amputating the end of EE's little finger just behind the fingernail. No regulations were violated.
The employee was running shuttle car on the 1WHG Section. The employee stepped out of shuttle car, left leg buckled and EE fell. All EE's weight came down on EE's left leg. The miner sustained a lower leg injury. The employee did not violate any laws or company policies and was wearing all the appropriate PPE.
Employee was climbing off excavator and slipped in the mud on the ground. Causing a small knee strain.
Working too closely to the edge of a boom truck, employee slipped off the side. Sore ribs
Driver did walk around on truck after leaving scales & step was slick getting in truck & employee slipped & fell on tailbone. ER dr. says bruised tailbone & go back to work in 2 days.
A belt mechanic was changing out a top roller when the belt unexpectedly started and put tension on the come-a-long being used to raise the belt. The come-a-long got wrapped around the injured miners hand causing lacerations to EE's right hand and thumb, and a fracture to EE's index finger.
Employee slipped and fell in mud putting wheel chocks down, felt left knee pop
Standing on a 2' pedestal was going to grease a fitting on the motor, went to turn body and foot got stuck on grating and went over backwards landing on cement. Broke left hand.
Standing on 3 foot ladder hanging curtain when a piece of the rib rolled out and hit ladder knocking EE off and EE landed on the ladder and pieces of coal.
The Roof Bolter was advancing up to install the next row of bolts, two pieces of rock fell between rib and installed roof bolt, first rock struck them in head knocking them to mine floor second struck them in the right knee causing contusions and swelling.
Loading boulders into employees truck, rocking the truck and hurting their back, mid back pain.
Employee was hanging high voltage cable for a power move and felt strain in EE's lower stomach area. Employee continued to work until surgery on 06/29/2019. First actual day away from work is 07/01/2019.
Employee was pulling self up on drill. As employee was pulling self up, employee felt a pop in abdomen
While unwrapping water hose employee tripped on hose and fell on right knee causing injury.
Employee was using a ball pen hammer hitting a packing gland when employee felt pain in shoulder.
Upon exiting the lube truck, employee stepped in loose gravel causing EE to twist EE's right ankle.
Employee was exiting water truck when employee spotted a snake in the area. Employee hurried to get back into the truck and sprained ankle while entering.
EMPLOYEE WAS HELPING WORK ON A BROKE CONVEYOR CHAIN WHEN A CO-WORKER HIT THE CHAIN WITH A 4 POUND HAMMER. WHEN A PEICE OF STEEL CAME OFF THE CHAIN OR THE HAMMER HITTING THE EMPLOYEE IN EE'S RIGHT EYE. THE STEEL IS STILL IN THE EMPLOYEE EYE AS OF TODAY 06/04/2019
Employee reported on 6/10/2019 that on 5/29/2019 while loading cable on pump car, they felt a sharp pain in their neck. Became LWD on 8/8/2019 due to surgery.
EE driving a 7 person diesel vehicle on the 6 North Track. Communicating with another operator of a 4 person diesel vehicle outby to drive toward each other to talk. EE parked vehicle at 45 blk to allow the other to approach. As the outby vehicle approached and began to slow, it slid into the parked vehicle causing EE to bump their knee against the throttle cylinder.
The belt on the power screen was stuck. While trying to free the belt it came loose causing the fastener holding the belts together to fly free and cut employee's upper left leg.
The miner was driving a haul truck. When the miner got out of the truck, they tweaked their back. Lost time accident as of 5-30-19.
Employee was pulling down a clear curtain and felt something pull in EE's left upper arm. Employee started missing work on 7/29/2019.
Employee was helping wash slope when employee fell on foot hurting ankle.
While climbing up into the drill to move it, employee grabbed the handle with right arm and used left arm to grab the steering wheel and felt something pull in left shoulder.
Employee walking across track at the top of the slope to load oxygen and acetelene tank when they slipped and fell, got their boot off and ankle was swelling put ice pack on then ankle and took them to the ER.
Employee was stepping up on D10 Dozer to check oil for starting of shift when EE twisted left knee.
Employee was servicing a loader and as employee went to climb into the lube truck, felt a pinching in left groin area. As the employee continued with a few more services, the area became more painful the more the employee got in and out of the lube truck.
Closing the bottle of the mill, the closing lid catches the finger of the left hand.
Employee was lifting materials out of service truck when employee felt a strain in lower back
Employee stated they were operating the #507 outby scoop while cleaning the belt line, EE hit the rib with the operators' deck and the rib broke free and rolled onto them, striking them in the back of the head and upper back. The rib measured 30"" x 62"". The scoop the employee was operating does not have a canopy.
While employee was carrying a bag of rock dust employee felt a pain in lower back.
Walking through the plant got debris in the left eye that irritates the eye
Employee was walking down stairs and felt a pain in EE's lower abdomen.
Employee was riding in the front passenger seat of an 8 person belt ride. A rock fell out between roof bolts and struck them on the head and chest. They sustained a laceration to the face and fractured ribs.
Watching head drive and reached around guards to remove a piece of coal. Thumb got caught in belt on #3 head drive.
Employee was shoveling coal onto the belt conveyor when EE felt a pain in EE's left shoulder.

The employee was sitting inside the cab of a company motor 4B when another motor and dust pod car became uncoupled rolling back and striking employee's motor. When the cars collided an object flew off one of the cars striking the employee on the right side of head causing a laceration and a concussion. The employee was wearing all the appropriate PPE.
EE was assisting to tear down the screen box. EE got thumb caught between the deflector plate and the screen box.
Employee was trying to get the winch rope out from behind the ram of the scoop. When the ram of scoop was pulled back, employee got fingers caught between the bucket and ram of scoop causing injury.
While belting up, EE pulled rail apart and smashed middle left finger between another rail resulting in fracture and stitches.
A fully trained miner was walking around the water car when the miner contacted a piece of wire mesh and caused a laceration to left ear. The injury was not the result of any compliance issues nor were any protective guards involved.
IE was helping to pump cribs and stepped in void that was filled with concrete. The area hadn't set up yet. IE stepped into void area sinking to knees causing IE to get stuck. Other employees helped IE out.
Injured Worker had hand on top of stone; other worker accidentally hit with a hammer while trimming the stone. Pinky finger on right hand was smashed.
Employee was pinned by the boom of the Continuous Miner in the 2 south right section. Injuring employee's back and ribs.
Right Hand punctured with a chain conveyor. The operator wanted to remove a broken bag from the conveyor.
Operator exited a portable restroom and while walking back to equipment stepped on a rock and rolled right ankle resulting in a sprain and avulsion fracture
EE was assisting to cut a support tube section out of a conveyor when a piece fell on the EE, knocked them to the ground causing a fractured tibial plateau fracture and a 3rd degree burn to their leg.
Pulled 3-10' cable bolts off of back of roof bolter and carried them up beside roof bolter, felt pain in upper back. Set a row of roof bolts and pain got worse.
Employee was working on LW move. When EE went to get in EE's bucket, a piece of pan line slid and struck employee on the foot causing injury
Worked Full Duty till LDW 8/5/19 - Loading a shot on uneven ground, as EE turned from one hole to the next EE lost footing on a rock, hurt right knee.
Employee was operating a water truck on Grapevine North Contour. Employee was going up an incline to the pit. The truck engine died and began to roll backward. Employee said it rolled about 75 ft. and they steered the truck toward the highwall. The rear tire rolled up on a berm and it flipped on its side.
Employee was standing on steps of 16 M grader cleaning windows and the door came back on EE knocking EE off the steps and hitting EE's head on corner of grader blade. Required 12 staples.
The employee stated was leveling some belt structure when a piece of rock fell striking the employee on the left side of EE's face above the eyebrow, causing a laceration. The employee sought medical treatment and received fourteen sutures. The employee sought medical treatment the following day and was taken off work.
EE had an umbilical hernia prior to employment with Rolling Rock. EE has no idea what EE did to aggravate it. Hernia is larger and needs repair. Due to aggravation and enlargement it is work related and does not fall under short term disability.
EMPLOYEE USED A HOT STICK TO TRIP #4 REGRIND MILL BREAKER. EE HEARD A BUZZ AND RAISED EE'S HANDS TO SHIELD FACE.
While changing batteries on scoop, the battery forks on the scoop got caught on a battery tray as operator was bowing them down. The forks slipped off the battery tray causing the operator to be bounced around in the operator deck. Lacerated operator's ear resulted in stitches.
Failure to LOTO while unbinding an auger at the lime silo. Employee applied a pipe-wrench and failed to remove it when the auger was being bump tested. Employee was struck on the left side of face causing an open fracture of the jaw.
A fully trained miner was carrying tools from the belt to the track when EE tripped on a hump and twisted left ankle. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee stated was rock dusting #8 rt entry when foot struck a rock causing employee to stumble while trying to catch self. Employee states during this motion employee strained lower back and left knee. Employee sought medical treatment on 5/21/2019 and was placed under doctor's care.
Employee stated was dismounting the haul truck, as EE approached the ladder on the front bumper EE fell to the ground EE lost balance and fell.
IE was removing a piece of metal out of a chute. As IE pushed the piece of metal off of the side of the belt, IE felt something pull in IE's left shoulder. Still waiting on the Dr's report to see what is wrong with IE's shoulder.
A rock slid off the top of the miner hitting me in the hand.
Employee was assisting in dropping a transmission into a 993K loader. They bent over to move some hydraulic hoses out of the way and felt a sharp pain in the left arm and shoulder. However, the employee stated they had recurring pain in this area for over a week and could not pinpoint this injury to this particular instance. This is still under investigation.
Truck driver was exiting truck in parking lot. Steps fell off truck. driver fell onto steps receiving minor scrapes and bruising. was released from hospital with no injuries. went back to see dr on 5/22 and it was determined they had cracked ribs.
The employee was lifting a car lid when EE felt a slight pull in EE's back. EE filled out initial incident paperwork, but did not seek treatment until 6/20. This incident turned into lost time on 6/21.
5200-346 Chute - barring overhead in chute, felt pain in right shoulder.
Employee was at the face cleaning the lifter hole with a fin hoe preparing to install collar pipe. As EE was using the hoe a section from near the center of the face sloughed down towards EE striking EE to the ground on the left rib.
Employee fell while dismounting a D6T dozer injuring right shoulder.
Employee was up on the man lift. Took a pipe to try and bang sand off roller while the belt was running. It pulled the pipe in along with arm. EE pulled arm out, and that pulled skin down to wrist from elbow. Employee was taken down off of man lift and air lifted to hospital
Employee worked normal job on May 16, 2019. Everything seemed normal that day. Employee went home at 3:00 PM. At 8:37 PM employee called supervisor stating that employee is in the hospital with broken left hip. See Exhibit A.
Employee was walking by PTO from track bolter to motor when PTO failed and sparked striking them in the knee pad and back of leg. The injury resulted in a burn to the upper right leg. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency was not a factor.
Employee lifted arm to work on air lock and as employee raised arm employee felt a pop. Employee reported the incident two days later on Sat. May 18th 2019. Sent employee to hospital and has restrictions
The employee was traveling down the intake slope on a personnel carrier when the employee experienced service brake failed. Employee's dynamic brake was functioning but was gaining speed, employee steered the vehicle in to the right rib to reduce speed. After contacting the right rib the vehicle over turned.
Employee was washing rollers with a water hose. At the same time EE was monitoring a skirt board, which was rattling around, the end of the hose got caught in the sausage roller and pulled both hands between roller and the belt. Employee was trained and was wearing all required PPE.
Injured employee stated EE was operating the left Fletcher bolter on the #1 section, stated that EE was installing 10ft cable bolts in the intersection between entries 3 & 4 at #251 break, stated EE was 6ft in depth for a 10ft deep hole, stated that when bolt partner dropped pot it caused stress on EE's steel causing it to break and therefore strike EE's left pinky finger.
The employee started a hole with a peanut steel and put the starter steel in the chuck. When the employee reached for the peanut on the bottom employee's shirt sleeve caught the lever causing the head to drop down onto employee's hand causing a fracture.
Employee was loading tool box from tool cabinet to the mantrip. Picked up tool box from cabinet and turned around to load on mantrip and felt a sharp pain and burning in lower back.
The employee was changing the tire on a roofbolter and while breaking the lug nuts on the tire the employee felt pain in their abdomen. The employee was diagnosed with having a Hernia. The employee is continuing to work at this time.
Employee strained EE's back while transferring roof bolting supplies from a trailer to the roof bolter. EE continued to work with frequent pain and finally started missing work on September 28, 2020.
Injured employee (EE) was checking the oil level in the longwall shearer ranging arm. EE's right boot was in the face conveyor chain and as EE turned to exit the work area, the boot got caught in the chain causing EE to twist right knee. EE has pain and soreness in knee. EE didn't violate any regulations, has training and was wearing required PPE.
The employee was working under/next to the screen when a rock fell off of the top and hit their shoulder. The employee worked through the injury, but missed work on the 17th when they were seen by our provider and put on restrictions. Lost time as of 5-17-19.
A fully trained miner was rubbing EE's eye when EE scratched it with something that apparently was in it. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee initially reported pain upon exiting a light vehicle away from the mine site on 5/15/19. Later the employee reported that the injury must have occurred on 5/14/19 while EE was working on a tire, but cannot point to any specific time, place, or event where injury took place.
A miner received injuries while they and two coworkers were repairing a belt splice on the 1A conveyor belt. The miner had positioned themselves on top of the conveyor belt when it started.
I was watering roads when I noticed a flame, I immediately set the fire suppression off and started using extinguisher and tried putting fire out, then was taken out.
EE was struck by a continuous miner cable that had been caught by a shuttle car after being loaded. The cable knocked EE down onto the mine floor hurting EE's left knee. A MRI showed that EE had a fractured tibia.
EE stepped off of a flat car on left leg, and hyper extended knee.
2ND shift was splicing stacker belt at 6:50pm. Injured was standing behind another co-worker nailing. Co-worker was starting a nail and it ricocheted out, striking injured in corner of left eye.
EE got CKD on socks on 5/13 and was instructed to cleanup & change socks. EE did not change socks. On 5/15 it was discovered that EE had alkali burns around both ankles (in the crease where the socks gathered). EE was sent to the company Dr. and taken off work for 3 days to heal.
The employee was pulling the miner water hose through the belt regulator when EE became tangled in the hose then EE felt a pop in EE's right knee
Employee was in the engine compartment working to replace the head gasket on #8 mantrip. EE was prying on the pulley with a pry bar. The pry bar slipped causing employee to fall down into the engine compartment, striking elbow against the battery box. Employee is trained and was wearing all PPE.
While using a utility knife to cut a piece of plastic, the knife slipped causing employee to cut left hand below thumb.
As the 16 person mantrip was leaving the unit, it turned left to go outby. Employee was sitting on the left side of the ride and had their left hand on the outside of the frame. The ride struck the coal rib and smashed employee's left hand. Employee suffered two broken bones.
The mining trainer was attempting to help another miner hang wire while bolting a round when the miner operating the jackleg shut leg feed air off to grab another driver and left the machine up in the split-set. The machine fell from the bolt striking the mining trainer in the thumb. The mining trainer was seen a PMC and diagnosed with a fractured thumb.
The employee was hauling coal to the preparation plant on the haul road. The truck went up onto the berm and flipped truck over onto its passenger side. The employee was not wearing seat belt.
Employee was exiting a 4400 rock truck and slipped coming down the steps and fell breaking a bone in left hand.
Injury occurred on 5/9/2019. EE did not seek medical attention until 7/11/2019. Was tightening vice and pulled left shoulder.
Employee was pushing a rock into the conveyor of EE's car because it had pulled down some cables. When the weight of the rock shifted into the car, EE's finger was caught between the mine roof and the rock resulting in a hairline fracture to the tip of EE's middle finger and 9 stitches. Employee worked up until 5/13/19 when EE was taken off work.
Injured employee was operating the longwall shearer, headgate side drum mining towards the headgate. EE looked away from the shearer for a brief second & piece of coal/rock assumingly came off the drum and struck EE on the right hand. EE didn't see the piece coming at EE. Hand became swollen and painful. EE didn't violate any regulations, is trained and was wearing required PPE.
EE had finished shift and was walking to vehicle to leave. The lane coming from the office to the parking lot is elevated and graveled. EE said ee was not paying attention and ee slipped in the gravels and fell on hand. EE sustained a crushing injury.
IE was sitting in a shuttle car and reaching back trying to clean out a compartment to change a hose. The IE felt a ""pop"" in the left shoulder. Modified to an LTA on 07/16/2019.
Employee stated ee was traveling across the section when ee stepped on a slick spot where the shuttle cars were traveling, causing ee to slip and twist left knee. Employee sought medical treatment and was placed off work due to a strain.
Building a block wall, got it about 9 foot high when it fell over on me.
A fully trained miner was B-Bonding a wall when the B-Bond came into contact with miner's arm resulting in a rash. The injury was not the result of any compliance issues nor were any protective guards involved.
EE was installing splice plate onto hopper section of silo (B) TK402 access via ladder boots were muddy due to rain, employee slipped and fell about 4'. Fall resulted a lower leg injury employee was taken to hospital, x-ray determined a fractured tibula. Silo construction/mine development.
Employee was standing on the catwalk walkway that had loose coal on it. They attempted to hang chain hoist and twisted they fell down to the walkway on same level twisting left leg, knee and ankle.
Employee was exiting the mantrip after arriving to the slope bottom when EE placed EE's left foot on the barrel connected the throw arm of the switch causing EE's ankle to roll which resulted in a fracture of the distal end of the left tibia. This incident is being reported as a Lost Time incident.
A fully trained miner was using a hammer to hit a pump trying to unclog it when EE missed the pump and struck self in the leg. The injury was not the result of any compliance issues nor were any protective guards involved. NOTE: Employee did not begin missing work until 5/11/2019.
While working in the G1 #1 entry, employee was side bolting on tube side of miner, when a rock fell from top and struck employee on neck and back of head. Employee was diagnosed with a concussion.
The Miner lost footing and slipped falling awkwardly on their knee. The area was surveyed for tripping hazards and none were present.
EE was working on the clinker breaker. They were trying to remove the clinker breaker hood when D ring that the comalong was attached to broke off and struck them on the left leg. There were supervisors on site and one was right next to them. EE had just turned around when struck on the left leg. EE was taken to the hospital and was determined they had an open fracture.
Employee stated that while rolling up a section of conveyor belt they felt pain in the groin area. Employee was diagnosed with a hernia.
The employee and several others were recovering high voltage cable from the 1st left panel freeze fork when EE's foot was caught under the high line sled as it was being pulled by a scoop.
IE was riding on a track mounted manbus off of the section at the end of the shift. As they came up to a switch, there was a locomotive sitting in the track trying to get into the switch. As the operator tried to stop, the manbus made contact with the locomotive & jarred the manbus. IE's ribs struck the side plate & bruised ribs. IE is supposed to return to work on 5/15/19.
The employee was on the third floor of the preheater tower checking an access door to the orifice. A production crew was preparing to shoot cardox onto the preheater tower and were unaware that the employee was checking an access door to the orifice. When the production crew detonated the cardox, hot air burned the employee.
IE was hanging roof bolter cable. As ee went to hang a section on an orangs hook, ee missed the hook & as the cable came down it put a strain on right shoulder. EE didn't miss any work until 5/10/19.
employee was working from their knees and when they stood up they felt a sharp pain.
Employee slipped on steps and slid down to the bottom of the stairs hurting left ankle.
Contacted on 5/08/2019 by a OTR trucking company that one of their drivers had allegedly been hurt on our site. The driver did not report the injury to anyone at the time of the incident so details are limited.
Tool got caught in a head pulley resulting in pulling EE's arm into head pulley. Both arms became entangled in conveyor before de-energizing conveyor belt.
Employee was carrying and stocking new conveyor belt rollers in a crosscut when employee felt a pain in lower back. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
EE was cleaning windshield of loader when ee twisted and leaned, reaching to clean the middle of the windshield. EE felt a sharp pain in shoulder which progressively got worse. EE was seen at the ER, diagnosed with a strain to the shoulder. Follow-up resulted in the ee being put off work.
The employee was shoveling mud from under a conveyor belt. The employee twisted to toss a shovel-full of mud off of the shovel when the employee's back tightened up on employee. Back strain. Became a lost time accident as of 5/7/2019.
Employee was struck by a rib board and received a contusion to the left side of the chest, when the Rubber tired diesel mantrip that the employee was a passenger in turned too sharply while backing out of a crosscut causing the rib support (rib board) to come into the mantrip.
Fully trained ee #1 was helping loading skids of block into the scoop bucket. Fully trained ee #2 was operating the scoop and went to reposition the scoop to square it up to the skid of block. Employee #2 swung the bucket toward the rib, pinching employee #1's foot between the scoop bucket and a steel beam laying along the rib. Employees were wearing all required PPE.
Employee was walking down stairs. The boot caught a stair and injured the knee as the fall took place. 3 pts of contact was inplace
The employee was roof bolting off of the continuous miner when material fell from the mine roof and stuck the employee on the right side of the head, neck and shoulder causing pain. The employee was wearing all the appropriate PPE and did not violate any laws or company policies.
The Employee stepped into a rut and onto a rock and twisted their right ankle and fell to the ground.
Employee was going through a man door and slipped on a rock twisting back. Employee returned back to work until EE was placed off work by EE's doctor on 06/28/19.
A fully trained miner was using foot to hold an emulsion line against the rib while being pulled and the line caught a track tie causing it to jar and strain left knee. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee stepped in a hole resulting in EE spraining ankle. Cause of this incident was employee not paying attention to the surroundings. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency may have been a factor.
Removing tire from truck tire went to fall jumped out of the way landing wrong on foot.
Employee was jacking a man bus on the track when employee pushed down on the bar the jack slipped and caused employee to give away injuring lower back.
A fully trained miner was getting a dust hose off a scoop when the bucket was lowered and EE's foot was caught under the bucket. The injury was not the result of any compliance issues nor were any protective guards involved.
Pulling a curtain out of the gob and received a hernia.
Injured was pulling cable off EE's shuttle car at the end of the shift and strained EE's shoulder. Injured was placed on restricted duty for EE's injury.
Employee was assisting in repairing the track rails and while EE was carrying a track jack, EE stepped into a hole and twisted EE's left ankle. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
Employee was trying to unclog line and lost control of it when it was re-pressurized which blew shotcrete toward EE's face and into EE's left eye.
Employee was lifting 16' roof mats onto a scoop when they felt the left shoulder pop twice which resulted in pain and a sprain/strain injury, employee was seen by a specialist on 5/14/2019 at which time employee was taken off work. This is now being reported as a Lost Time incident.
In repairing screener, EE released the open/close lever and in doing so did not anticipate the weight of the equipment. The equipment began to close rapidly and caught EE's finger between the screener and channel iron.
The employee was shoveling dirt from under a conveyor belt. As EE twisted to throw a shovel-full over EE's shoulder, EE felt a pop in the shoulder. Shoulder strain. Became a lost time accident on 5/3/2019.
The employee was lifting the elevator doors on the 6th floor of the preparation plant, when ee felt a pain in right shoulder. The employee is trained, did not violate any laws or company policies, and was wearing all the appropriate PPE.
The employee was walking down 3 Belt and tripped. The employee struck right elbow on the mine floor injuring self. The employee was properly trained. The employee did not violate any laws or company polices and was wearing all the appropriate PPE.
Employee rolled EE's ankle on a piece of coal but finished EE's shift. Sought treatment on 5/8 after walking on it for a week, was released but was referred to orthopedic for closed fracture of fibula, saw ortho on 5/14 and was put in walking boot and taken off work.
Dozer operator stepped out of equipment cab onto tracks, slipped and fell on track fracturing 3 ribs.
While 6467A Continuous Miner was tramming through the intersection of #6 entry, the injured employee was also hanging the roof bolter cable from the mine floor. As the miner came closer, a miner bit on the drum inadvertently came in contact with the injured employee's right foot, causing a puncture wound.
Employee was removing miner bits with two hammers. EE taped up the heads of the hammers when striking together. When EE did hit them part of the claw split off and entered into the left arm. EE was seen at a Hospital and they could not get it out. EE had Surgery and started missing on 5/20/2019
Employee was driving up a ramp after being loaded, felt like the truck may have bottomed out, reported to the supervisor, had EE checked by first responder. Employee waited until the next day and sought medical attention, was taken off that shift due to lateness in day before shift.
Injured was replacing a bushing on a Symons 4.25 crusher. As they descended the fixed caged ladder and stepped to the ground they landed awkwardly and suffered back pain. EE sought medical treatment,was diagnosed as lumbar strain. Given pain medication and muscle relaxers and instructed to rest for 2 days. EE's total time working at this quarry was one day.
Employee was pulling a dust hose west main 3 belt line when employee stepped on un-even ground and twisted left ankle.
Employee stated was attempting to pick up a roll of belt with the loader when the belt slipped off the forks. When the belt slipped off this caused the loader to teeter throwing EE into the canopy injuring neck. Employee sought medical treatment on 7/10/2019 and was placed off work under doctors care.
While installing a mandoor, employee was driving a wedge with an ax and lost concentration and struck hand causing a fracture to EE's right little finger. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was climbing onto water truck and the swing step swung out on them causing the employee to strain their knee
injured was building a stopping and stood up and felt a pop in EE's knee. Went for an MRI and on 6/7/19 results were given that EE had a tear on EE's meniscus.
EE was attempting to clean out material built up in the tail pulley. While trying to free up the drop door dropped down knocking over the EE and landing on their leg causing injury.
A new employee was injured repairing a fluidizer lance. The employee dropped the lance and quickly grabbed it and pinched the left pinky finger between the lance and collar. Their finger was cut and also crushed. This job was being completed ""manually"" and there was no work procedure to follow.
Employee was descending stairs when they missed bottom step causing them to fall and fracture right foot.
Employee misjudged the last rung of step ladder, missed rung and went to floor. Twisted right knee with wrench in pants pocket. Dislocated knee cap, eventually diagnosed with tear in ACL, given light duty work restrictions
On 7/18/2019 the employee provided the mine with a doctor's note stating that they could not perform job duties due to possible injury to the right knee that the employee states occurred on 5/29/2019 while cutting electrical conduit and kneeling without knee pads (knee pads are made available at the mine site).
The maintenance team was installing a new screen when a 6 inch square steel tube supporting the screen tipped and struck the miner's hand catching the hand between the tube and the catwalk. It resulted in a fractured left thumb and laceration on left hand.
Employee was squatting and felt pain in knee
Employee was cleaning out an elevator when EE's elbow began to hurt.
Employee slipped on klinker ball causing them to jerk their leg in an awkward motion. This resulted in a sprained left leg injury.
Employee was driving a water truck and hit a pothole. Employee stated the drivers seat bottomed out causing lower back pain that radiated to the chest.
Employee was pulling out a rolled 3 inch by 20 foot rubber hose from a wooden crate. Employee stepped backwards and tripped on another hose. They fell backwards to the ground.
Employee slipped on a step while performing maintenance on a palletizer. Footing was not secure, access step is narrow, rushing/frustration, some talc on the surface from bag that broke. Sprained knee with possibly more damage - waiting on MRIs - out of work.
Employee was walking back to a Dump Truck and a tree 12 inch diameter fell on employee fracturing the fibula lower leg and a thoracic vertebra. Employee is able to move all extremities. Spinal cord is not damaged.
EE was attempting to pull a strap from under a pallet. A forklift lifted the pallet, releasing the strap and causing EE to fall backward into a stack of pallets. EE sought medical attention for elbow pain where EE was advised to rest for the next shift until x-ray results were determined. EE was given a prescription as treatment.
Removed undercarriage to remove wheel and studs. Lug nut stuck on wheel stud, put pressure on wheel to remove lug nut. Wheel moved as lug nut came off. Pinched hand between impact gun and wheel axle. Stitches (4) laceration on palm and bruised hand.
Breaking Stone and fell between the stones and one stone rolled back onto EE's leg
Employee sustained minor left knee strain when knee twisted while stepping off of D8 Dozer. 06-10-19 - Due to minor experience of pain in left knee, employee was taken in for further evaluation and treatment; thus assigned physical therapy.
EMPLOYEE WAS PUSHING A BLENDER INTO ITS CRADDLE, AND FELT A POP IN RIGHT SHOULDER. EMPLOYEE DISLOCATED R SHOULDER, WAS SET IN LOCAL ER AND WAS RETURN TO WORK ON RESTRICTED DUTY.
Employee was trying to dislodge a rock from the primary crusher with a 30lb jack hammer when a piece of rock struck EE in left wrist and arm knocking EE backwards.
Miner rolled an ankle on the platform above silos whilst walking across.
Injured was tearing down belt structure at the S4-C drive area and got a foreign body in EE's left eye. The foreign body had to be surgically removed from EE's eye.
Repairing cone crusher, tramp cinder piston, drop out right hand.
An employee was installing dust collector filters, stepped on the baskets and twisted left ankle.
Installing light fixture in admin. EE lowered JLG liftpod, unhooked harness and turned toward left and felt knee pain.
Employee was dragging a coiled up air when they felt a pop in the left shoulder. Employee went to the doctors and had an MRI and will eventually need surgery on the shoulder.
Injured was working around the M10 drive area and slipped and fell. When fell to the ground employee dislocated right shoulder.
Employee operates a wheel loader - The seat in the loader has air ride - the seat was starting to not hold air - when it would lose air it would take about 10 min to air back up - so while mining the person jarred their back and had to leave work to see a doctor. Other people had ran this loader the week before with no complaints about the seat not airing up.
Employee loaded parts into a truck. EE was putting on gloves and dropped one on the floor. EE bent over picked up the glove and felt pain in their back. EE told supervisor, finished shift. The next morning called in and said was not able to report to work. We sent them to the doctor on 5-15-2019 and the doctor has placed them on restricted duty.
Employee sustained a crush injury to the left middle finger while removing hardware that secures the top of the crusher to the base of the crusher.
Teammate was using a 1"" impact gun to install lugs on wheel and lacerated left thumb
EE was lifting and moving the supply line for a pump, upon repositioning EE's body, while holding the hose EE's right knee popped causing EE to collapse to ground. On 7/15/19, requested to see an orthopedic and on 7/19, employee was seen by the doctor where EE was diagnosed with a meniscus sprain with possible tear, an MRI ordered, and restrictions given.
Employee had hands full of rollers while walking up conveyor to change out rollers - tripped over hole in catwalk - landing on left shoulder/neck/back - employee notified supervisor and welder - area was roped off with caution tape until welders made full repair
Employee was helping carry an oxygen cylinder when it slipped from their grasp and landed on their left foot just above the steel toe. Next day they couldn't put the safety boot on due to swelling.
Officer was hurrying to ensure there was no unauthorized access at the gate. Stepped off curb, twisted ankle. Step is 7 inches tall. Boots were laced, but loose. Officer has sprained ankle.
Employee was preparing to start EE's shift. As EE was stepping over a curb, EE tripped & landed on EE's right knee. Employee was taken for medical evaluation, placed on Restricted Duty 5/10, & scheduled for MRI. MRI revealed torn quadriceps, scheduled for surgery. First day lost time 5/29
Employee positioned their upper body between the belts to knock a rock loose from tail pulley and belt using a hammer. When rock was freed stored up energy caused the belt to move in an unexpected forward motion catching the employee's hands between the belt and the pulley. Employee locked out the breakers but conveyor was not blocked from motion.
Employee was team lifting a nitrogen bottle. The employee slipped and fell. Employee put a hand out to catch fall. The nitrogen bottle landed on thumb. All correct ppe was being used including impact gloves.
Employee was removing a blow bar on the crusher using a hammer and swung the hammer to strike the wedge to dislodge it, but missed the striking target and got finger between the hammer handle and the door of the crusher. Causing right index finger to bleed a little and swell a little. Was given general first aid at onset, but later went for treatment.
Getting off an excavator and slipped and fell on a rock. It was rainy and slick.
P5 best stopped loaded with rock causing the drive belts to burn off. Employee was shoveling/raking rock out from underneath the belt and felt a sharp pain in EE's left shoulder.
Employee was descending stairs at carbon columns and a heel caught on the second step from the bottom of the staircase, causing the right ankle to roll. Employee attempted to catch themselves with the left foot and rolled the left ankle in the process. Both ankles were sprained and x-rays were positive for broken bones.
Employee was walking down the stairs on the plant when EE lost EE's balance and fell. The fall resulted in a broken rib and dislocated shoulder.
Driver was delivering Copper Concentrate to FMI Rail hopper. It was drizzling rain. EE opened the tailgate of the trailer to make sure load was completely delivered. EE slipped and grabbed onto the trailer to keep from falling. EE's middle left finger got slammed in the tailgate resulting in 9 stitches and fracture.
While standing on a ladder breaking loose bolts for the air dryer on a 793C haul truck. The employee strained the left shoulder during the process.
Employee states that on 5/6/19 they were moving heavy metal and pulled a muscle in their left leg and groin. Employee continued to work the next scheduled shifts and did not report the injury. The employee was out on leave until October when they stated the injury was work related.
Employee was hang the miner cable along the rib in the last open crosscut in the 1st North development section when a piece of coal measuring 18 inches long by 18 inches wide by 8 inches thick fell from the rib and struck ee in the left knee.
Employee was installing drive belts on the #3 ball mill feed pump. While rolling belts onto the sheave, caught fingers, fracturing ring finger. Taken to ER, given restrictions.
Employee was blowing dust off clothes at end of shift with a clothes blower and felt a pop in EE's back as EE bent over.
Maintenance employee was working at the BC-08 slide handle near the cement tank. Employee moved an 8-10 lb guard and felt a pop in right shoulder.
Employee was removing a guard feed box when guard weld broke, and guard swung back onto EE's left thumb resulting in a fracture.
Employee was working from the filter press maintenance platform when ee fell. The employee was taken to hospital, where it was determined ee sustained a lower leg and back injury. The employee was treated and released the same day.
Production mechanic was changing out the sleds under the impact zone of conveyor. Mechanic flipped the sled frame over with assistance from contract mechanic to finish installation. During this process mechanic felt a pull and then noticed a knot around EE's groin area.
EE was unplugging discharge chute on dryer #2 when hot material broke loose striking the EE on back of neck and ear causing EE to jump from an elevated surface, in doing so EE suffered sprained right heel and minor burns to neck and ear.
Employee was operating a front-end loader and jarred left arm in rough terrain. Left arm muscle strain. Did not become reportable until after Clinic follow-up on May 10, 2019.
While inspecting air conditioner on Haul Truck 209, employee locked out transmission and left truck running. Employee slipped while using a ladder and EE's left pinkie finger slid between the alternator guarding contacting the alternator cooling fan causing a laceration/fracture
Employee was completing preventative maintenance on a conveyor motor, when an unexpected start up occurred due to incorrect lockout, causing a motor shaft coupling to come loose and contact employee's right forearm resulting in a laceration and fracture.
Employee was in the process of cutting drip line on leach pad with a razor knife, while cutting the plastic line the knife slipped, making contact with the left forearm, causing a laceration to the forearm,requiring stiches to close.
Left rear wheel came off of cart and lost control of vehicle, braced for impact when cart fish-tailed. Initially, a first aid incident, follow up Dr. appt July 1, placed employee on restricted duty.
Employee was operating a skid-steer in the wet plant and struck a concrete curb. This resulted in the employee striking the lap bar of the skid-steer in the abdominal area, and striking their left knee on the interior cab. The employee was wearing a seat belt at the time of the incident.
While pulling up to get on dozer, employee reached for grab bar and felt sharp pain in their shoulder
While operating portable plant employee pinched finger on grizzly bar leading to a minor fracture and 6 stitches on right index finger. Contributing factor was not following the standard operating procedures set (task training).
Employee grabbed onto edge of belt cover (sheet metal) to pull self up for inspection. Sharp edge of metal cut into employee's multiple right fingers. Most of the cuts were superficial, but one cut resulted in exposed tissue. The employee failed to use gloves and a nearby ladder.
EE placed right hand underneath Conveyor 1 which was in operation at the time. EE's hand was pulled between two rollers resulting in abrasion and skin removal. There was no broken bones nor were the nerves damaged.
The miner was assisting in changing out the north hoist rope, the hoist rope was stopped in a small sheave. EE put EE's boot on the hoist rope and it moved knocking EE onto the ground striking EE's elbow and hand.
While taking the shoe off of a core tube with a parmalee wrench, employee slipped when the shoe broke free, employee fell to the ground and the parmalee wrench came free and fell on top of employee's left index finger causing a small fracture to the first knuckle.
While lifting a 35 pound bag of cat litter to be run through a splitter for gathering test samples, EE felt a pulling/popping sensation in EE's right shoulder area. Able to work- Sprain of shoulder rotator cuff- no lifting 5 -10# - take meds. -home exercise program -refer to physical therapy.
Employee was wedging a block while standing on block already wedged. When the line split, it caused chain reaction of rough backs to fall, causing employee to go over the edge of the block. EE fell 3 to 4 feet, causing dislocated right shoulder & inferior glenoid fracture, also abrasions to both forearms. Shoulder was re-set, employee's arm is in sling. As of now, not lost time.
Injured strained back while lifting a Roof jack. Injured was placed on restrictions for the strain.
Miner was splitting a piece of stone with a chisel and hammer when a piece of stone broke off and fell on EE's left foot. EE's left foot received a hematoma bruise.
While cutting a piece of pipe with a cutting torch, pipe fell off injuring fingers between it and another piece of pipe.
Employee was walking on the leach pad when they stepped in a soft spot causing soreness in their left ankle.
EE using a pry bar to move a stuck rock, pry bar slipped and mashed left ring finger. EE received 5 sutures in finger.
Employee was replacing guard on 50 HP motor with help of second employee. During the next work day employee complained of left shoulder pain claiming it was from replacing the motor guard.
Employee was in the boneyard looking for a part. It was raining out and their boots were muddy. Employee pulled themselves up to look into a screen frame when they slipped and hit their knee on an I-beam causing the injury to the knee.
EE was taking tire down from second level of rack. Tire bounced and struck EE in hands. EE was seen by Dr. and diagnosed with Fractured finger and placed on restrictions.
On 5/24/2019 at approximately 3:45pm. EE was power washing rubber tire loader when EE over extended the wand causing it pull back causing the wand to travel across EE's boot cutting boot and causing 2nd degree burn to right foot.
Employee is a Splitter Operator at our indoor Chilton Manufacturing Facility. Employee was making a cut of the stone. When the cut of the stone was complete, the stone bounced back causing a finger to be caught between the two stones as the left hand was up on the rollers.
Crew was tripping pipe at the drill site and were asked to vacate the site due to concurrent customer operations. During prep to leave the site, employee was pushing on pry bar with EE's left leg facing forward. As EE pushed on the drill pipe, EE felt a pop in EE's left knee and fell to the ground due to a complete tear to medial, patella, and lateral aspects of knee.
Mine worker was picking chunks of mud from the feed conveyor belt. Worker felt a strain and soreness in upper wrist/forearm area.
While traveling on haul road, haul truck operator hit a pothole jarring lower back. Initially a report only, the employee reported on 5-23-19 and did not require medical care. Employee went on days off and when returned to work on 5/28/19, went to ER for strained muscle. Began 3 days off on 5/24/19, followed up with dr, restricted duty.
A welder received a shock while welding a phone enclosure on the top deck of the Galloway. The employee was transported to the ER and was released to light duty.
EE was moving the control box while EE was setting up saws. EE was walking backwards, lost EE's footing and fell backwards. When EE fell the control box EE had been carrying landed on EE's hand, breaking EE's thumb. EE went to clinic, was released back to work. Went to follow up appointment Tuesday, May 28th. Has lifting restrictions of 1 lb., continues to work, no lost time.
Employee was removing a pressure switch, when Employee released switch, a plastic part of the switch flew and hit EE's right middle finger.
Two employees were pulling (120 lbs.) bucket off of forks of a forklift sliding it onto the chain to our reclaimer. While pulling the bucket one employee felt a sharp pain in lower back.
A miner was cleaning a cordless grease gun when it exploded and caught fire, burning the miner's left hand
Employee works at a stone quarry. No other information on conditions contributing to accident.
EE was working with the utility crew in 454-1 taking down waterline. As EE was taking the clamp loose the pipe separated unexpectedly, falling and catching EE's right hand between the pipe and the rail of the tele-handler basket. Employee was given ice by safety then taken to the doctor to have hand examined. X-Rays were negative but was given <5lbs lifting restrictions.
The employee rolled their left ankle as they stepped out of the service truck.
Miner was descending a ladder when EE slipped off of the rung causing EE to drop to the next rung before EE was able to regain EE's balance causing discomfort to EE's right knee.
The contracted employee slipped while weed eating on an inclined bank. The slip resulted in a fall down the incline of approximately 4 Feet. The contracted employee stated that EE strained EE's back.
Employee was handling cutting edge and finger was caught between edge and bucket.
Employee was driving loader too fast on bumpy road and the jolting motion caused a strain in EE's lower back

Employee was replacing a blower on line A when EE was struck on the head with an 18"" aluminum pipe wrench that had been dropped from an upper level of approximately 35'- 40'
HAND RAIL GAVE WAY AND EMPLOYEE GOT forearm caught on broken area and ended up with laceration to right forearm.
Miner was lifting a 50# gear reducer up onto a shaft that was in front of EE. While lifting EE heard and felt a snap in EE's right forearm in the muscle. Amount of damage is undetermined at this time.
Miner injured their right hand and received six sutures to the right thumb when a railcar collided with a platform gangway that was not mechanically secured in place.
Employee was hanging cable when rubber tire on mantrip hit foot.
Employee slipped and fell while exiting the oil containment area fracturing a rib.
EE was using the grinder when EE felt the grinder started to feel like it was becoming loose. The kill switch was located directly near the grinding wheel and guard. EE was slowly moving the grinder down towards the ground so the wheel wouldn't hit EE. As EE did the wheel came off and so did the guard.
The haul truck trainee went to enter the cab on the passenger side of the haul truck when their kneecap shifted to the left and popped back into place on its own.
Clearing out a stoppage of the crusher, employee placed forearm on the frame and peered in the open access doors. One of the access doors was jarred and fell onto employee's forearm, fracturing right distal radius and ulna.
The miner was lifting a tire and straining abdominal muscles.
Contract employee was setting a steel jack down and it slid together pinching EE's index finger. Cause of this incident was awareness of this employee performing the task. Compliance with rules/regs, mining systems, PPE was not a factor. Miner proficiency may be a factor.
The crew working at the 14/15 Chute at Metcalf Crush Convey loading metal liners into a truck. The affected Employee picked up a liner that had a bur on the bottom side hidden from Employee's sight, as Employee reached out to pick it up the burr cut the left palm of Employee's hand through the glove. Employee received 4 stitches.
While employee went to open flop gate in the screen chute, gate created resistance, employee cleaned around gate, went to push gate down, lost balance and left and middle finger got caught between closed flop gate and wall of chute. Tip of middle finger was crushed resulting in tip of bone to have multiple chips and skin removed with exposure to muscle, fat and tip of bone.
The employee was cleaning off the waste belt when EE went to turn the water off, and stepped into a small dip and twisted EE's knee and felt a pop. The employee was taken to an offsite clinic and seen and released back to work and followed up with a clinic and at this time was given work restrictions.
EE bending over to pick up a piece of drill steel and felt pain in low back area. EE went for medical treatment on 6/7/2019 and was placed on restricted duty at that time.
Employee was moving concrete like cover away from wall to retrieve an item from behind it, the cover shifted and pinched middle finger on right hand between wall and cover resulting in a laceration.
Employee report pain in their back when the bed of the haul truck came down after dumping a load of rock to the crusher.

The technician was dismounting a forklift when EE missed the bottom step and fell, landing on EE's left hand and wrist. The result was a fractured wrist.

Employee was attempting to clean an area at line 2 cooler side door. While using an air lance the air hit a hard piece and forced dust back up into face. The material went around glasses into eyes. Flushed eyes continuously for 30 minutes. Followed up a few hours later and injury had not improved; so sent employee to receive medical evaluation.
Employee was in haul truck when they claimed a boulder shook the cab while being loaded. Causing a back strain.
Employee was climbing down from a conveyor frame to the catwalk (approx. 3 ft. below) when they lost grip. As they reached out to catch themselves, allegedly they grabbed the handrail, their body continued in downward motion, and caused a strain to the left shoulder. Employee was examined and released from the local ER. Follow up visit the next day at Occupational Clinic (May 14)
Employee was changing the tracks on the main feed hopper when finger got stuck between two metal plates
Removing a piece of brick that was wedged in the end gate of the dump truck. End gate shut catching employee finger between latch and bar of end gate. 19 stitches required.
EE was entering the operators cab of a CAT 982M wheel loader. As ee went from the door way of the cab to positioning body in front of the operators seat, ee felt a pop in lower back. Then when ee sat down, ee had severe pain in right hip (sciatic nerve pain).
Attempting to mount the chain case of a motor grader while lifting a starter into place. Slipped in mud, fell forward and pinched right little finger distal between the starter and chain case.
Employee bent over to lift a bundle of empty cement bags that were no more than 40 lbs. EE picked up the bags using back and felt a sharp pain from back right side to groin.
Changing screen on mid wash screen deck, belly crawling, felt pull in upper left side of back
Was cleaning out old mains, 42 inches high and muddy, while tossing piece into scoop bucket it fell short into the mud, when picking up felt pinch in shoulder.
EE was removing a cutting edge from a loader bucket with an air impact when the bolt spun, grabbed EE's glove and ripped through it, cutting EE's left hand palm and wrist.
EE attempted to move a bolder that had rolled into path of loading haul trucks. EE felt a pop and a burning sensation in left bicep as they pushed the rock. EE was able to operate the loader and continue work without restrictions. A burning sensation continued and surgery was required. EE was not able to return to work as of Jan 2 2020.
An employee was attempting to clean a raw material chute with a bar when a gate closed which pulled the bar in causing the gate to close on EE's left arm. This resulted in a fracture to the left arm.
Employee was driving 06 Haul Truck down Hokie Pit Road, where other equipment had been stuck in the mud on the previous day. There was ruts in road where dozer had turned to pull out stuck equipment. Employee hit ruts, causing employee to bounce and seat suspension bottomed out, jarring employee's back. Employee was taken to clinic and released.
EE was attempting to extend a come a long to the span EE's task required EE to. While free spooling the come a long, EE's right index finger was pulled into the guide sprockets which caused the EE to sustain an avulsion to the tip of the right index finger. Work restrictions were issued as a result of injury.
EE was in the adjustment area on the jaw crusher trying to slide the block behind the toggle plate cover to center it up. The cylinder on the porta-power was at an angle and when pressure was applied, it kicked out and hit EE in the face and left arm. This resulted in 3 stitches between the nose and mouth.
Working to dislodge blocked material miner injured back.
EE wrote was pitching mud out of k-4 chute and rock rolled down and hit backside of hand and wrist. Doctor put EE on modified duty on 5/8/2019. EE is not to use right arm until follow up appointment.
Plant wash water hose charged causing nozzle end to hammer/jerk, spraining EE's wrist.
A miner suffered a sprain of the right shoulder joint while pulling on a wrench. The miner was changing screens at the Chip Plant when the incident occurred. The miner reported the incident on May 8th however, did not request medical treatment until May 14th.
Employee stated EE was working to remove suction line on the bottom floor. Employee stated EE was standing in coal fines when EE turned to reach for a wrench but EE's foot remained stuck causing EE to twist EE's knee and ankle. Employee reported incident on 5/8/2019 and sought medical treatment. Doctor placed injured employee on restrictions due to sprain ankle until 5/13/2019
The employee was trying to adjust the ""T"" handle on a dial indicator when EE felt a pop and pain on top of EE's right middle finger knuckle.
Strain to upper arm, possible tear to bicep muscle. Mechanic was repairing a broken coolant line on a haul truck. Was reaching to remove the fitting and pulling on the wrench when they felt something in upper arm pop. Was taken to the ER and was given a 10 pound lifting restriction and scheduled a follow-up with a orthopedic doctor.
The injured worker was opening the door to the Wirtgen Continuous Miner with left hand. The miner was parked on a slope and the heavy door opened quicker than expected. Employee reached for and grabbed the door with the right hand and it twisted their right thumb and wrist.
The employee was walking down a set of stairs when they misjudged the last step and jarred their back.
Employee was walking down the stairs and slipped, catching self on the handrail with left arm. Employee sought medical care on ee own that night and we had ee see another on the 7th.
Employee struck left arm on poly pipe as ee lost balance and fell off workboat.
While employee was washing glass bottles, a bottle slipped out of their hand breaking and causing a laceration to their left wrist.
Injured was handling materials for building an undercast and strained back, on 5/6/19 injured was seen by Doctor and placed on restrictions for the thoracic lumbar strain.
Miner was having problems with rt. elbow, appears to be from a previous surgery non-work related. On July 3rd the mine was informed by WSI the inflammation in the elbow was in a different location where the surgery took place year ago. This appears to have been cause, while pushing on a dragline cable during the re-socketing process in May. Currently on restricted duty
EE was on the 4th step of an 8 foot ladder, drilling on a lube reel box to install a buggy whip. The drill bound up causing the ee to lose balance and fall to the ground. EE was seen at the ER, diagnosed with a fracture to the radial bone. EE was released to restricted duty.
The miner claimed that the air ride in the seat of the 824 John Deere loader was not functioning and the ride control was also not functioning causing lower back to ache.
Employee was doing maintenance on Dryer-100 where employee smashed the tip of thumb between a steel plate and drive chain.
Employee noticed a gradual onset of shoulder pain that they relates to demolition of overhead electrical conduit using a Sawzall. Ladders were utilized instead of aerial lifts due to tight spaces. First medical evaluation 5/9/19. Surgical repair was done 8/1/19. First time loss was 8/1/19.
The Operator of the 640 Haul Truck was loading on the right side of the 16 shovel when on the third dipper a boulder struck the top left bed rail causing the 640 Haul truck to shake. The motion of the truck shaking caused the Haul Truck Employees head to whip back and forth.
The Employee was trying to remove a guide roller by hitting it with a 4 lb. sledge hammer to loosen the guide roller. EE swung the hammer with both hands on the handle striking finger between the hammer handle and the structure. Fracturing the tip of right index finger.
Due to rain, employee stated employee slipped in mud and injured back
EE FELT PAIN IN LOW BACK WHILE BENDING OVER TO PLACE A WHEEL CHOCK UNDER WHEEL OF LOADER. EE PLACED ON RESTRICTED DUTY AFTER MEDICAL TREATMENT ON 5/1/2019.
A miner was diagnosed with an inguinal hernia on July 29, 2019. EE believes it occurred on or around May 1, but was not able to provide a specific cause or incident. Surgery is not currently indicated but EE has been placed on restricted duty. EE reported the injury to us on July 29.
The individual was lifting a box beam when they felt a sharp pain and a pulling sensation in their hip.
An employee cut EE's finger on a conveyor frame while cleaning beneath the conveyor. The employee received three stitches on EE's right index finger. Employee returned to full duty, without missing any days of work and without work restrictions.
There was a 4' strap installed on the roof that was bent down. Victim cut hand trying to bend the strap back. EE received 4 stitches
Employee was in the 4 left break loading out shot up rock and trimming rib line when a piece of unknown debris was thrown from the cutter head of the miner striking EE in the face causing a laceration below the left side lower lip requiring stitches.
Employee was taking apart the take up on the 6A Cross conveyor when the new pulley that was just behind the employee started to roll, so employee put a hand out on the shaft of the pulley to stop it. The key hole on the shaft of the pulley was touching employee's pinky finger when it rolled with employee's hand, which is when employee believes it cut the left pinky.
Employee was moving a rock away from the coal rib, when the employee started moving the rock the rock slid down the coal rib smashing right middle finger. The finger was cut and required 5 stitches.
EE was driving a grounding rod into the ground with a hammer, and their hand slipped up towards the head and they hit their right index finger. Received (6) stitches.
Employee was struck by an airline hose when it slipped from the hose clamp holding it. The employees hard hat was knocked off and EE received a laceration to the top of the head. Employee received three staples and was released to work full duty.
Employee was trimming trees on the entrance road to the quarry when the employee slipped and struck face on a tree limb.
Employee was stapling slip sheets on pallets and EE moved/lifted a pallet and strained lower back.
Fletcher Roof Bolter ATRS knocked down a rock from the roof. When the rock fell to the bottom it landed on another rock and broke apart. One of the smaller pieces flew up and struck the bolter helper in the chin. Employee was standing approx. 12' behind the last row of bolts. Employee received a cut on the lower lip/chin area which received 4 stitches.
Pry bar came loose while trying to remove a screen striking IW in the mouth and damaged IW's tooth
Employee had just completed installing rib bolts when EE traveled back to the area of the drill inch control tram levers. EE stated EE went to bend down under a piece of wire mesh that was bolted to the top. The top of the wire mesh was hanging down approx. 4"" in the area & caught the top portion of EE's right ear lobe resulting in a laceration. The cut was cleaned & closed.
EE was tucking pants into boots after break and felt a pop and sting in ring right finger.
Miner was changing the tail wheel of a conveyor, miner was beating the wheel with a hammer a small piece of debris flew off and got stuck in the miners skin located in the stomach area.
Maintenance employee was using a forklift to align a brass bushing in a socket for the 4 1/4 crusher. EE stated that EE's hard hat had fallen off several times. EE leaned over and struck head on one of the forks. EE was not wearing the hard hat at the time. This resulted in an injury requiring four staples. The employee was treated and returned to full duty the same day.
Employee exited scoop and rock fell from rib striking employee on the left knee causing them to fall forward. Incident was reported immediately to MSHA. Employee received full work release from medical facility.
The employee was rib bolting on the 1WHG Section. The employee scaled the mine roof, and was closing the rib drill. Additional material fell out of the mine roof striking the employee's left hand lacerating the pinky finger. The employee was wearing all the appropriate PPE and was obeying all laws and company policies.
EE was cutting piece of skirting with safety knife. Skirting moved and EE cut left forearm with knife resulting in laceration. EE received 3 stitches resulting in MSHA reportablity.
Employee was doing cell maintenance when ladder shifted causing the employee to strike their shin on the ladder.
Miner was using a sledge hammer to separate a bucket tooth from a shank which had broken off the bucket. On the fourth swing, a small shard of metal broke off the bucket tooth and struck the miner in the left leg below the knee. The metal embedded into the shin.
Employee was removing a roller out of an unused scavenger magnet. When the retaining bolt was removed the employee thought it was still supported. As the primary bolt was removed the roller fell and landed on the employees finger causing a laceration.
Employee was operating a rubber tire dozer when they hit a pothole causing back pain.
REPORTABLE ONLY DUE TO SUTURES - Employee went to sit down on tool box and caught EE's right index finger between the lid and box causing fracture to tip of finger requiring sutures.
Employee was removing air lines from the starter of the HT517 when EE slipped off of the second rung of the ladder falling to the ground. EE hit elbow cutting it.
While ee was using a drill press in the shop, they cut two of their left hand fingers requiring stitches.
The employee stated was laying track when the rail slipped off the jack smashing left hand. The employee sought medical treatment at a hospital where employee received three sutures to middle finger and four in ring finger and diagnosed with a fracture of the tip of the finger. This incident is being reported as medical only.
While bolting, employee felt something contact middle back. EE looked back & saw a small rock & continued to work for several hours. Later EE mentioned the incident to partner, who took a look at EE's back, at which time EE realized there was a laceration. Seen Site medic, referred to ER. Staples were used to close cut & full work release issued making this medical reportable.
Employee was loading hydraulic jack onto skid in warehouse when employee caught finger between jack and skid resulting in a laceration. Cause of this incident was lack of employee concentration. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency was a factor.
Employee was servicing a 988G loader 129' from a highwall. A root cluster with dirt and rock fell from the highwall and, upon impact, projected the dirt and rock into the haulroad towards the loader. A rock 2"" x 3/4"" came through the engine compartment and glanced off of the employee's head. Employee received 3 staples to close the laceration.
The employee stated was in the process of cutting a piece of a lifeline when the knife slipped, causing a small laceration to EE's right index finger. The employee sought medical treatment and received five sutures. This incident is being reported as Medical only.
Team Member reported eye irritation resulting in conjunctivitis from exposure to airborne foreign particulates following the use of a grinder while wearing safety glasses and face shield. Examination by ER physician did not find a foreign object or corneal abrasion in the affected eye.
Miner was helping with a derailed rail car. EE reached under railcar to move support. While pulling hand out from under rail car EE's left hand contacted a bur on the metal of the car caused a laceration to the top of EE's hand near the thumb. Miner was not wearing gloves at the time.
The employee was wearing googles, full face shield, and 1/2 mask respirator, while cutting an access door in a tank. Something got in their right eye, causing it to have redness and a scratching feel.
Employee was rinsing the mud off of loader and was walking backwards pulling the water hose towards the back of the machine when they tripped over a wheel hub assembly that was on the ground. They reached backwards to brace themselves for the fall, and in doing so their hand contacted the top of the hub assembly causing a cut to the palm of the left hand.
Employee was using the clothes cleaning booth. While the air system was in operation a piece of debris went around the employee's safety glasses and went into eye.
The employee was pulling on a wrench and the wrench slipped hitting the employee in the bottom lip and front bottom tooth, causing a small laceration to the lip and a small chip to the tooth.
Employee was pulling on wrench, felt tear in left arm from elbow to shoulder.
EE was checking fuel levels and mis-stepped off of a ladder. EE did not completely fall, but EE did hit EE's knee on the rung.
Employee braced self with left hand on the rib as EE walked past a shuttle car. The employee lacerated left pinky finger on the rib mesh that required eight sutures.
MPCS division independent contractor was installing floor grating. EE had cut a piece off and returned to cut more and stepped onto a piece of metal buried in the sand. The pointed sharp edge punctured the boot and caused a laceration under the left big toe.
The employee was using a pipe wrench to loosen a pipe. The wrench slipped from the pipe, causing EE's left hand to be cut on the middle knuckle. The employee was wearing gloves at the time. The cut required stitches, which were done at Concentra Health in Little Rock, AR.
Miner had EE's finger pinched between a training roller and a piece of belt structure when a chain broke that was holding a come-along.
Employee stated was tramming the right side miner into the face of the #8 entry when EE reached to take the strap off and a piece of rock fell and hit EE's elbow. inby station 3+50. Sutures were required.
Employee stated EE was climbing into a screen deck to replace a screen when EE cut hand on a sharp rock.
Employee was drilling and cutting a metal panel and rubbed their right eye. Later that night they saw something in the eye and had to go to clinic to get it removed.
Stinger Drill Rig operators Kevlar sleeve caught the feed control lever and energized the drifter which moved and pinched the operators right hand between the shank and the drill rod.
Contractor's employee smashed fingers under a brick while unkeying brick ring for removal from kiln.
Crew was working to remove old fence from entrance area. Sections of fence were cut down and carried to the bucket of a backhoe. The backhoe was then used to remove the fence from site. While placing one section of fence into the bucket, the cut end of the section of fence caught the employee in the forearm/wrist area and caused a laceration.
Employee was walking on catwalk at scalper and tripped. When EE reached out to catch self EE bent right pinky back on the guardrail. This resulted in a minor sprain for which the employee has a splint on the finger for one week.
employee was changing pin with porta power. Pusher metal shot out, ricocheted off dozer and struck them in the face. They received 30 stitches for the wound.
The employee struck EE's head against the mine roof. Contact with the roof caused EE's hard hat to come off which allowed EE's head to contact the roof. The EE was transported to hospital where EE was examined and treated for a laceration to the top of EE's head which required sutures to close. EE was released to return to work full duty.
The employee was using a pry bar to straighten the step on the 1250 komatsu trackhoe when the bar slipped and hit their hand causing a cut on the right palm.
The employee was walking to the port-a-john when plastic (trash) wrapped around their leg and tripped them. They hit the gravel causing cuts to their hand and forehead. No lost time, but sought treatment on 5-20-19.
Employee was carrying an electric motor with assist of 1, from the stairs to the crusher shack. Employee states hurt back while carrying motor.
While cutting wash screen spray bar boots, the injured employee inadvertently cut themselves on the front of the right thigh.
While moving a supporting block beneath a conveyor assembly under construction, the block was left crooked. Employee attempted to straighten the block and the conveyor assembly dropped unexpectedly and caught the meat of EE's right little finger. The crush injury caused a 15 cm laceration.
Worker obtained a laceration to the left 5th digit while cutting a metal plate. EE was wearing all appropriate PPE including leather gloves. EE did receive sutures.
While unhooking a choker, the choker swung back and hit employee in the mouth. Causing a false tooth to come loose.
Employee stated that when they attempted to turn and duck underneath the canopy of the bolting machine, they struck their face on a roof bolt plate that was hanging over the edge of the bolt tray. This caused a laceration to the right cheek that required sutures. The employee did not miss any scheduled work due to this incident.
Exiting locker connex, employee stepped into a small hole, rolling their left ankle.
Employee was closing a gate valve, at the head box of primary screen. While closing the valve, EE struck left elbow on the flange of the top rail of the head box, causing a laceration that required two sutures. Management was notified on 5-20-19 that the employee received sutures due to this incident.
Employee was pulling a 6 inch drisco line into heading using a 2 yd. loader. As EE came around a corner & straightened out the drisco line contacted EE's back pushing EE forward causing EE's mouth to contact the loader. 2 sutures were required, x-rays positive for a chipped tooth, a full release was issued making this a medical reportable
An employee was attempting to free multiple small wires by cutting the zip tie that was holding them together with a utility knife. Employee received a cut below the left thumb that required three stitches.
Employee was attempting to remove plug on left front spindle to take an oil sample when the ratchet with an 1 1/2 socket slipped off the plug pinching the employees left little finger between the shaft of the ratchet and the rim. Employee was wearing gloves.
Technician was changing bags in baghouse. After removing approximately 20 bags the technician reached down to pull out another bag when the technician felt pain in right shoulder. The bag became stuck on the way out causing the strain. The incident became reportable on 6/14/19 due to assigned physical therapy.
EE was working on the A/C unit on Bus 2 EE removed and re install Blower Motors when EE turn it back on the bus got full of dirt from the unit and it got into EE's eyes prescription eye drops given.
A miner was using a pneumatic die-grinder to prepare the steel surface on a dump truck for welding when the miner felt something go into eye. The miner flushed eyes several times & finished shift. The next day the miner experienced discomfort in the eye & sought medical attention. A foreign object was removed from eye & miner was released to work with no restrictions.
Pinched Finger under rock - R Ring Finger
Employee was disconnecting flexible water hose with cam lock fittings. While trying to pry the fittings apart with a tool, one of the hose ends struck the employee on the nose.
Employee was lifting hood off of D11 CAT Dozer, Hood shifted and pinched left index finger between the hood and the holding bracket for the hood. Broke index finger.
Bolter operator was using the boom/pot to make room in the mine bottom to be able to attach the second steel. The starter steel was in the roof being held by bolter operator's left hand. The canopy struck the left hand pinching the area between the thumb and index finger between the canopy and steel. The bolter operator had no fractures and received three stitches.
Employee was going to strap rock that was stuck in cone, the plant man ran the cone cleanout backwards causing the rock to fracture. Part of the rock struck employee in the face.
The miner was loading round when a small rock fell and caused a laceration to left cheek. EE was seen by the site medic and referred to Pioneer Medical Center to have cheek evaluated. EE received sutures and was issued a full release.
EE was operating a Caterpillar 992C loader in the South Pit. As they were driving the machine, they reached up with left hand to adjust the airflow coming out of an overhead air conditioning vent. As they reached up, their hand hit the adjacent vent and cut the middle finger resulting in sutures to repair.
Employee was using a utility knife to cut a roll of wide electrical tape into half when knife slipped and it cut employee's left thigh. 4 sutures were required to repair the cut.
3 employees and 1 contractor were working on the freshwater pump. They uncoupled the pump from the pipe to turn the valve around when the barge started to sink on one side. The employees/contractor climbed off the barge and swam to shore. The injured employee cut L middle finger while climbing off, requiring 2 stitches.
Was outside top of slope picking up a battery and felt pain in lower back. They gave the employee a back brace to wear.
Employee wiped off face while wearing gloves. Gloves had residual metal on them from work. A sliver of metal got into the employee's eye.
Maintenance employee was using an oxygen/acetylene torch cutting the heads off bolts. While cutting the bolts the employee feel heat on EE's lower legs. When EE looked down there was flames on EE's right pants leg. Employee was diagnosed with 2nd degree burns, no lost time or restricted duty.
IE and another EE were dislodging the manifold from the water line which was hung from the top. IE was holding the manifold while chain was hooked around it and other EE was hitting it with a sledgehammer. Once the manifold broke free, the chain unhooked and the manifold unexpectedly fell on IE's hand causing IE's right thumb to hit a belt structure bolt.
Employee was removing an exterior weld bead from a 10"" HDPE pipe using a cold chisel. The employees hand and cold chisel slipped contacting EE's left forearm causing a laceration requiring 3 stitches. Employee was immediately released to work with no restrictions following medical treatment.
Employee was cutting piece of rubber when the blade came out of the folding utility knife EE was using causing a laceration to EE's right index finger. Employee received 4 sutures. Employee was wearing PPE, hard hat, safety glasses and gloves.
Employee was removing a chain from the scoop when the load shifted causing the chain to contact finger.
Employee was using a stationary wire wheel to remove metal burrs from a short unistrut section, +/- 5"" long
While striking a metal bucket shroud to remove it with a hammer, a sliver of metal came off the hammer and embedded into the welder's left leg.
The employee was cleaning pond over flow drain. The pipe inlet has an expanded metal barrier to catch large debris from entering the inlet. The employee was reaching down to remove large debris from barrier and was bit on EE's right index finger by a venomous snake hiding under the debris. The worker was wearing rubber style gloves when bit.
Operator was operating a haul truck on site on asphalt when the fuse panel cover opened and fell on operator's head.
Employee stated EE's foot wrench slide out from under EE's tray and when EE reached to push it back in while lowering EE's boom, the boom pushed the wrench down on one end and up on the other catching EE's middle left finger on the tip against the bottom of EE's tray.
While replacing a cooler on the Atlas Copco D65 drill, the Miner was underneath the drill securing the bottom bolts. Dirt fell from the machine and struck the Miner in the face resulting in a a paint chip in eye.
EE was cutting plastic shrink wrap off of a pallet of gear guard parts. EE cut the plastic with a pocket knife using right hand. EE reached down to pull the plastic from the pallet with left hand. There was a piece of the plastic that had not been cut. EE pulled the plastic with left hand, and when the plastic broke free left thumb contacted the knife blade.
Reportable only due to a suture. Employee was using a hammer when a piece of the hammer broke off hitting employee in the right forearm. The piece of metal was removed and required one suture.
Contractor was removing steel from the coal rib. While EE was bringing the steel out, EE pulled the mast back towards EE to reconnect the steel and felt sharp pain in the middle finger on the left hand. The bottom of the mast hit EE's finger lacerating it against the back of the frame of the slide. This resulted in 10 stitches to close the laceration.
The employee was cutting a piece of belt with a utility knife, the knife slipped and cut their upper right leg and they required sutures
Two employees lifted a metal gear box to discard it in a metal waste bin. One of the employees got their right middle finger pinched between the gear box and waste bin. Employee suffered a laceration that required 3 sutures.
Employee placed hand on boom of liner handler. Handler retracted pulling employee's finger into pinch point resulting in a pinched finger
5200-184 Rock on face - went to pull it off, slipped and fell forward, rock slipped off face and caught middle finger of right hand between two rocks.
IE was getting set up to make a belt splice & was removing structure. As EE lifted EE's side of the 60"" top roller assembly, the employee on the other side pulled on it & IE dropped the top structure & pinched EE's left middle finger. EE required stitches in the end of EE's finger.
During bolting activities the injured miner was attempting to get out of the way of a rock and was either struck by the drill of another miner or fell into bolting supplies that were leaning against the shaft wall. The miner suffered a lacerated nose and scraped elbow.
Ee was using a wrench to loosen a bolt, when the bolt came loose the wrench came back and struck the ee in the mouth, fracturing two front teeth. Released to full duty, tooth numbers 8 and 9 will need repair and caps.
Mechanic was working on man basket of a dozer and was struck in chin by a bar trying to free locking pin, 3 sutures were required.
During the bolt up cycle an employee was tightening a split set bolt to the rock a 2"" by 4"" rock broke away & fell thru the wire mesh contacting EE in the safety glasses causing the glasses to push down on EE's face & nose resulting in a laceration on the bridge of EE's nose, sutures were required, full work release was issued making this a medical reportable.
Employee caught left middle finger between an oil bath and the frame of a Belt drive. EE's finger contacted a sharp edge on the oil bath and cut the finger. EE received 8 sutures to the left middle finger.
Employee was walking in the back area of the mine office when EE observed a black snake by the trash can. The employee bent over to pick up the black snake when EE was bitten on EE's right hand between EE's pointer finger and index finger. The employee did not miss any work and both agencies were notified by calling the hotline as a safety precaution.
On 5/11/2019 the employee was raking grates during the shift and then went to the top of K2 coal bin to clear a plugged chute. While bending over the chute with a rod that was used to clear the opening, the employee felt pain in EE's back. After prolonged pain in EE's back the employee sought medical attention on 6/19/19.
It was determined through a Part 50 Audit that this miner took a vacation day on 6/5/2019, that would have been reportable. We submit this in protest and to abate citation #9222853. Employee was operating a shuttle car when employee was struck by a piece of draw rock, that fell from between the bolts and struck employee on the head & neck.
While EE was roof bolting, EE had placed left knee on the mine floor and then was positioning self under the canopy to lower the foot jack. EE then tried to push EE's body higher and at the same time EE's right foot slid under the foot jack while it was being lowered. The jack came down on EE's foot. As a result, x-rays revealed a small fracture on the big toe.
Employee was standing on Shield 176 and was operating the pan push function, using the in-shield controls. EE left hand was resting on the gob wing. As the TG drive pushed forward, ee middle finger on the left hand was pinched by the gob plate as it moved closer to the shields during the push.
While loading a roof bolter with supplies, the employee smashed left thumb between two bundles of roof bolt plates.
The ee was attempting to free an obstruction from the rock dust hose by applying air pressure. The obstruction released, the employee lost control of the rock dust hose and was sprayed in the in the right eye with rock dust. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
EE cutting metal, as slag fell on top of the R/foot & burned thru boot leather onto foot, reported injury, declined med atten. On 5/29 emp requested to see Dr. due to foot not healing as expected. Occ Med noted infection and presc rx w/follow-up at wound clinic on 5/30/19. Returned to work no restrictions currently.
Crew was air drilling in a pit and were changing bits. EE's hand was caught between bit and drill rig resulting in a fractured bone in EE's hand.
3 mechanics were working on a screw and motor. when the last bolt was removed, the screw twisted around and bumped the mechanic and knocked EE down.
Employee of Toro Rock Products was moving boulder by hand and pinched middle finger between two rocks. Resulting in Stiches.
Employee was welding on a slide wrench. Employee had welding shield down and didn't notice the slide starting to move. Slide wrench fell on employee's left pinkie finger splitting open top of finger requiring 5 stiches. Employee was wearing thick welding gloves and proper PPE at time of incident.

Employee stated was experiencing soreness in left wrist. Initial treatment was first aid, on May 20 doctor visit employee was given a wrist brace to use while at work.
Employee placed their right hand in a pinch point location. Employee was replacing a universal joint on a light duty vehicle.
Crew was using a sledgehammer to cut a block and during the process employee was hit by hammer causing injury to the finger.
Removing wear plate. Wear plate bounced off wall of chute and contacted employee leg
A contractor employee was cleaning off a plate of metal to prepare for a weld to clear some dust from the surface, the employee used mouth to blow the dust off the plate when a foreign object entered eye. The employee immediately attempted to remove the object from eye with help form a coworker, but they were unsuccessful.
IE WAS REPLACING RAW COAL SCREEN AND WENT TO STEP OVER THE NEW SCREEN AND SLIPPED CAUSING EE TO RECEIVE A CUT ON LEG.
EE was gathering welding lead to weld in another area. EE stumbled and fell backwards from same elevation and struck EE's left elbow against a piece of steel. EE received 3 sutures to EE's left elbow.
IE and another EE were lifting a plate into position for the welder to install a lifting eye. While moving the plate into position, IE lost IE's balance causing IE to drop the plate and pinch IE's right middle finger. This incident resulted in IE receiving sutures.
Employee was in the process of squaring up a rock brow in order to drill for a shot after the coal had been undercut. As EE started to shear down in the rock, a piece of metal (carbon) projected back down the entry towards EE. It struck EE approx. 2"" above the left eye above EE's eyebrow resulting in a laceration requiring 3 stitches.
Employee was helping secure the screen from inside the screen deck. The screen was turned on and began vibrating material onto EE's screen. EE scraped and bruised EE's back as EE was exiting the screen.
Employee was walking while performing geologic mapping in a wooded area west of the Concentrator. While walking ee tripped over some brush and downed branches, fell onto a rock, striking left knee, which resulted in a laceration that required sutures.
Employee was preforming maintenance tasks on the cone while plant was shut down when ee lost footing on the cone discharge. EE fell from approximately 4 feet high onto shoulder. EE has had shoulder problems in the past and it does become easily dislocated. EE shoulder was in fact dislocated and ee was taken to Medical Center for treatment.

Setting up a hoist to lift gear cover off an engine. Injured employee was giving commands to mine mechanic who was running the hoist remote. Injured employee had said ""hold on"" but the mine mechanic continued to lift, causing tension on the chain and pinning EE's finger against a water pipe. The injured employee suffered a pinching laceration that required 12 stitches.
