A wind storm caused the CEI transformers on the site to lose power. The mine was evacuated via the Koepe conveyance. All employees were evacuated with no injuries or other issues. The power company (CEI) restored power the following day around 1:00PM.

The weekly examiner found this fall on his weekly run in G entry of the North B return at spad #5+99 The fall was approximately 20'long and 17'wide and 6'high. This area was mined in 5/1/2006 and was supported with 6' fully grouted bolts. No injuries or property damage. The fall is blocking the return as is being cleaned up and supported.

A roof fall was discovered on 1 South Parallel Mains at break 13. The fall was approximately 20 feet long. Ventilation was not impaired and no travelways were impeded. All approaches to the fall were breakered off.

An unplanned roof fall was discovered near Spad #11463 at the mouth of the West Mains 6 left bleeder panel. The fall measured approximately 20' long and 40' wide and fell just above the 4' anchorage. The fall occurred in the regular travel-way for the weekly airway examiner and was reported via hot line.

An unplanned roof fall was discovered by a mine examiner on #1 section, #5-6 intersection, @ #12 block. The fall measured 70'L by 18""W by 14'H.

The Pleasant Grove Elevator was down from 1:45pm to 3:00pm due to a computer malfunction.

The mine was shut down for the Holidays. There was no shift start time. No one was working. We had an 8"" rain storm. Water entered the pit and eventually entered the mine openings.

A roof fall occurred in the No. 4 entry of the 1st South East Main, 11 Seam. The fall was located in the pillar line between crosscuts No. 33 and 34 and measured approximately 18' X 20' X 6'.

An unplanned fire which was extinguished within 5 (five) minutes occurred on a 650 battery receptacle located in the F24 recovery section at the battery charging station located in #3 Entry at 3 wall. There were no injuries as a result of this incident. The right battery receptacle and the charger cable plug were damaged.

On 12/22/15 a roof fall was discovered in the 1 Left 3 East air course 1 crosscut outby the #2 seal. The fall was approximately 70' long X 20' wide X 14' in height. The cause of the fall was determined to be weathering.

Sunshine Mine lost power due to damage caused by a tree contacting main electrical service line owned & maintained by Avista Utilities. Loss of power occurred while miners were working in Jewell shaft. They were temporarily stuck in shaft till power was restored & they were hoisted to surface. Power was restored by Avista at 10:04am & the miners were hoisted to surface at 10:10am.

On December 21st 2015 the service hoist located at Camp Creek Portal went down with 12 people on the hoist due to the tail ropes coming in contact with the wooden beams at the bottom of the shaft.

As B shaft skip hoist was leaving the pocket, damage occurred resulting in the hoist being inoperable for more than 30 minutes.

During routine x-ray examinations, it was discovered that the rope on the supply hoist was bad. The hoist was immediately taken out of service and plans were made to install a new rope. There were no injuries or property damage due to the incident.

At approximately 5:00pm the Silver Shaft Hoistman was performing his checks when the hoist faulted and displayed a North Brake not Released Fault. No Miners were on either conveyance. Electricians found the North Brake Release Limit Switch was out of adjustment. Electricians corrected the problem and pre-shift checks were completed and Hoist was released to Production.

On our Updraft Drying Fan failed and we had a fire in our Electrical Room (EER1). 45 minutes prior to this we know the current to the Fan went way up, speed went way down, power draw went way down, motor temps skyrocketed and the fan vibration decreased.

Top of power pole was observed as smoldering with intermittant flames. Contacted local fire department and power company. Fire was approximately 12-16"" in diameter at the top of the pole and did not travel down the pole. Approximately 3 hours elapased before it could be verified as 100% out. MSHA hotline called at 11:32 following inability to confirm it was out beyond 30 min.

Roof fall was discovered in the inactive working of the old 3 main panel at spad location 1934 extending over toward 1940 between No. 1 and 2 entry. Fall was approximately 40' in length, 20' in width and 6' 1/2 in height. Area was developed 11/15/2011. Cribs set and timbers. Area dangered off. Ventilation controls were not affected. No injuries, no equipment involved.

While going back to do his pre-vehicle inspection report on his welding truck the employee discovered the welding truck adjacent to his was completely burnt as it appeared to catch fire between the afternoon shift and morning shift.

At approx 4:45 a problem was identified during normal testing of the production hoist main breaks. Trouble shooting began, repairs were made, and the hoist was opperational at approx 5:45 am.

Fire occurred on Tram Motor 24 at 5200-155 Raise. Miners unplugged battery power to motor and used extinguisher into motor . Miners then exited drift for the 5200 shop. Stench was initiated and mine evacuated

An unintentional roof fall occurred on the #1 Section #5 Entry 1 break outby spad 5818. The fall was approximately 25ft x 20ft x 6-7ft. There were no injuries reported. Two (2) mobile roof support jacks were involved in this roof fall.

At approximately 7:15 p.m., the Spice Lick elevator was taken out of service due to an electrical malfunction.

Electrical, loose Wire #1 contact. MSHA notified 8:25 p.m., evacuation of miners started at 8:14 p.m., repairs were made and tested at 8:40 p.m., miners returned underground at 8:48 p.m.

An unplanned ignition of coal dust occurred internally in a coal mill. There were no injuries to people, nor was there damage to equipment. This incident was called into the MSHA Reporting Hotline on December 10, 2015. Confirmation was received by the MSHA Henderson Field Office Supervisor. This is a follow up written report.

On 10 December 2015 at/about 11:30 PM, a rupture disk on No. 3 Coal Mill opened releasing pressure build up to system.

An unplanned roof fall was discovered at 9:30 am located at the 1st NW main xc 152 spad 124+43 #9 entry. The roof fall was on top of some power cables. Approx length 40' by 20' width by above 6' high.

A programming issue was found with the ventilation doors in the #1 emergency hoists making it inoperable. There was no damage to equipment, and no personnel were in the #1 shaft at the time of the incident.

A roof fall occurred on MMU-001 located at spad 408 in the number 2 entry 2 feet above the anchorage point.

The control cable was pulled off of the Portal 2 auxillery hoist man cage causing it to be down for over 30 minutes. The incident was called into the MSHA 800 Number.

North loading gate was caught on structure in the loading pocket. The mine was evacuated with no injuries or other issues. The issue was corrected and the hoist was returned to service.

A Roof fall was discovered in B-Mains x-cut 4 intersection of the number 2 entry. The fall was discovered on a weekly exam. The fall compromised our secondary escape way, all miners were withdrawn to outby that location. Report was called in to the one eight hundred number upon discovery.

The Crabapple slope hoist was not operational for an extended period of time due to electrical issues found during normal inspection. No employees trap or injured as a result of this incident

During the weekly examination, a reportable roof fall was found in F Seam Southwest Main Intakes in entry 3, XC-20. The fall was approximately 30' long, 15' wide and 7' high.

On 12/7/2015 while performing examinations, the examiner discovered an unintentional roof fall near the intake seals. There were no injuries, or damage to equipment. The fall measured approximately 4' x 4' x 16'. The M.S.H.A. was notified and an investigation was conducted.

Employee was entrapped in the pre-heat tower elevator for 1:30. There was radio contact with the employee and he was in good health. The elevator repair contractor was called and they removed the employee unharmed.

The Dry Ridge Elevator was not operational for an extended period of time. The rope grabber oil level became low causing a fault. No injury as a result of this incident

Air compressor went out. MSHA was notified at 12:16 a.m., evacuation started at 12:15 a.m., hoist was repaired at 12:28 a.m. hoist was tested and men returned underground.

A non injury roof fall was discovered in the #1 south panel in the #6 entry at 2375 & 2386 spad App 40ft long 18ft wide and 10ft thick. Neither ventilation or primary escapeway were effected. The affected area has been cribbed off and dangered off.

The service hoist at Portal 2 was taken out of service when a defect was noticed in the rope during a routine inspection. The rope was replaced.

One employee became entrapped on the Fish Creek elevator for 40 minutes. A faulty door switch was the root cause.

An unintentional roof fall occurred on the number 1 belt tail. The fall occurred in the number 4 entry at survey station number 63. The size of the fall is 12 foot 4 inches wide by 12 foot one inch long by approximately five foot high.

At approx 4 PM a drill operator found a misfire in u27 d tunnel face. The drill operator attempted to use an inappropriate technique and utilize the face drill steel to clear the misfire. This action caused the misfired cap to unintentionally detonate. There was no injury and minimal property damage. The drill operator's actions did not follow approved misfire protocol.

Elevator had stopped abruptly at approximately 45-50 feet from the top landing with production crew aboard. The braking system set automatically due to build up on rails.

The 8 Haulage elevator was inoperable in excess of 30 minutes due to one of the wedges catching. No one was on the elevator during this time.

On 12/1/2015 a roof fall was discovered on the 3 West Mains located in the #9 Entry at Brk 7 at spad 1699.

A non-injury accident occurred at this operation involving an unplanned roof fall. This fall was discovered on December 01. 2015 at approximately 06:00 p.m. The fall occurred in the area of spad # 10085.

Struck against a moving object

Preliminary investigation shows that the employee was fatally injured as the result of a collision with another haul truck. Investigation is ongoing. EE slid backwards and striking the cab of a parked Caterpillar 795 haul truck on the drivers side.

Sub-contracted tow truck was dispatched to tow super sucker truck. Boom of tow truck was being operated by tow truck employee #1 when it struck tow truck employee #2 in the head. Employee #2 was taken by ambulance to local ER for treatment where he later passed away. Most data unknown due to HIPPA and subcontractor status of deceased

Struck against a moving object

Employee was transporting material down slope A. Slope A belt shut off on a fault. Upon investigation of cause of fault, employee was discovered at the bottom of slope A.

An employee's hand was pinched causing the employees right index finger to be amputated at the middle phalange, the right middle finger to be lacerated and wrist to be fractured.

Contact with electrical current

Alleged incident-EE knocked #9cm breaker, was removing the cathead and received an electrical shock while their neck was against the cable. Incident is still under investigation. Cause is unknown at this time. Compliance with rules/regs at this time is not factor, incident involved equipment, job skills not a factor, miner proficiency may be a factor, PPE not a factor at this time.

Caught in, under or between a moving and a stationary object

Employee Was clearing topsoil with a dozer. A branch was entangled behind the blade. When the employee tried to remove the branch, it slipped, pinning the employee's right ring finger between the branch and the equipment. When employee removed his glove, the tip of his finger had been removed. Employee kept fingertip and glove with him and was immediately driven to the hospital.

Over-exertion in lifting objects

Employee claims he felt tightness in his abdomen while carrying a pipe. Employee was diagnosed with a hernia.

Employee twisted Left Knee mounting a dozer. He sought medical treatment for his injury on 2/29/2016 & began missing work the same day.

Struck against a moving object

Employee was driving to work in his personal vehicle on access road leading to mine. He ran into the mine's water truck on the access road. Employee suffered two cracked vertebrae in his neck.

Over-exertion in lifting objects

Employee was in the process of lifting a 16 foot I beam, along with a five other employees, onto the ATRS of the rib bolter when EE felt a sharp pain in EE's lower back. LTA as of 1/11/2016.

Fall to the walkway or working surface

Employee was walking across a frozen rutted area when he tripped and fell on his right shoulder resulting in a simple fracture to the upper humerus bone.

Contact with hot objects or substances

An employee sustained burns to the left side of his body. The dryer feed belt had gone down, causing an accumulation of 210 degree crystal and liquid. The employee was standing in front of the chute washing the material off of the belt. A blockage higher in the chute broke free and ejected hot material where he was standing, he tried to turn away but it contacted his left side.

Employee was walking in the reclaim tunnel, as he was walking he slipped on some ice and sprained his ankle.

The employee was walking on grating at a pump station, employee slipped and fell on some ice. This resulted in employee striking right knee on the grating. Employee had knee swelling and complications weeks later......(Through medical evaluations, it was determined to be work related on 5/20/2016).

Contact with hot objects or substances

An employee was starting to cut out a bearing on a pump barrel when the grease flashed in the bearing and a fire ball of grease shot out of the bearing and deflected off the face shield of the employee cutting the bearing and struck another employee standing nearby in the facial area with hot grease.

Over-exertion in lifting objects

Employee was shoveling material and as he was shoveling he felt a sharp pain in his back. Employee overstrained or twisted his back wrong and pulled a muscle.

Employee was walking down small flight of steps six steps total. Employee stated he misjudged the top step causing him to twist his right ankle. Employee stated he did not fall at any time. He was utilizing three point of contact.

equipment operator slipped on the steps of a 980H loader. Fell back tried to catch himself, resulting in a broken left wrist.

Over-exertion in pulling or pushing objects

Miner was using a chain fall to position the rock box for the 3 East transfer. He felt a pain in his left shoulder while pulling down on the chain fall trying to maneuver the rock box into its proper location. Given time off for shoulder pain. Was reaching overhead to pull down. No problems with mining methods, ppe, training or compliance issues.

Over-exertion in lifting objects

Contractor employee was performing service work on a continuous mining machine. The contractor was attempting to unpin the short side of the rub rail to clean the demister on the machine. When the rub rail was unpinned both sides of the rub rail came out at the same time. The contractor put the rub rails back into place and strained his back.

A trained longwall shearer operator was removing stone off the shearer and stone rolled onto his ring finger. 5 stitches

Rained earlier in the day, stone was wet. The worker used a hand grinder to scribe a line on stone. The disk shot out cutting the worker's stomach.

Strain of the calf muscle. We were setting a pump due to the extreme rain. He pulled his calf muscle while placing pipe. The slope (which was not extreme) may have helped cause the muscle strain.

Operator had dismounted the blade and was walking away from the machine when he slipped on the snow covered ground landing on his left knee. The MRI showed a tear in the cartilage

Employee was lifting a belt roller to put on a pallet and felt a sharp pain in his lower back.

Employee was walking to his tool box and felt a pop in his right knee.

EE was unclogging a plug when an object struck his head. Another employee found him knelt over & wobbling when he stood up. The employee went for help & came back to find EE gone. EE left the worksite & did not report the injury. He sought medical attention approximately 1 1/2 hrs later & reported a concussion through dr notes on 12/30. Investigation is ongoing.

Struck against a moving object

743HT driver passed a blade, stayed near the windrow, saw the 716HT approaching, so stopped. 716HT driver thought there was room between the 743 and blade to continue, but grazed the rock deflectors. 743 stated he was fine, went to the ER after shift, given a RX muscle relaxer, off work until clears D&A test.

Fall from machine

EE had just finished fueling his Komatsu PC400 and was climbing down to set his fuel nozzle in the track and lost balance and fell approx. 4' to a steel deck of the barge. He landed on the left side of his head and shoulder. Gateway Dredging employees observed a 3"" laceration and EE said he had pain in his left shoulder, rib and back area. Immediate decision was made to call 911.

Wheel loader operator stepped off the ladder, turned and twisted ankle. Caused swelling for approx. 48 hours, employee has returned to work and is doing fine.

Employee (bolter operator) was located in the #6 entry on the #1 Section (003 MMU)and was walking ahead of a fletcher RRII bolt machine and was struck by the bolt machine as it was tramming through ventilation curtains. Employee was positioned between the bolt machine and a Joy 1415 CM. Employee received multiple fractures to his pelvic area.

Contact with hot objects or substances

Because of poor illumination the miner could not see and did not use his flashlight. He used his cigarette lighter and the fuel barrel exploded, causing the miner to be burned. He was taken to a hospital with a trauma center for treatment of burns to his face, arm and hand.

Rigging sling broke, ripper shank fell and struck employee's feet

Struck against stationary object

While traveling along C2 drive on his Johnson, the employee struck his left arm on a wiper bar. This resulted in a dislocated elbow and fractured arm.

Over-exertion in lifting objects

The employee was carrying a bundle of 5 roof bolts to supply the bolting machine. When he lifted the bolts to place them in the tray, he felt a pain in his left shoulder. The following were not a factor: rules, regulations, mining equipment or systems, protective devices, clothing, skills, proficiency or training. Attitude may have been a factor.

Struck by flying object

Removing wing shroud on Letourneau L1850 and a small metal fragment broke off and struck him in the neck. This was originally classified as 1st aid because the doctor said leave it in it will heal fine and put a band aid on it. We encouraged the employee to seek a second opinion, he did and the metal was removed February 4, 2016, elevating the incident to a lost time injury.

Employee was welding at chest level. When he finished his weld, he lifted his welding hood and felt pain in his neck.

Absorption of radiations, caustics, toxic and noxious substances

Employee loading a truck on an extremely windy day and got product in his eyes due to a gust of wind.

Inhalation of radiations, caustics, toxic and noxious substances

Employee reported smelling an odor in the haul truck and feeling ill. She was taken to the ER, diagnosed with an acute headache, and taken off work.

The employee was using both hands to put his trolley pole on the wire. When the pole sprung upward, he caught his right hand between the pole and a beam. The following were not a factor: rules, regulations, skills, proficiency, training, protective clothing or protective devices. Attitude may have been a factor.

Fall onto or against objects

Driver was unloading pails of lubricant product for delivery. The pails were on a pallet. He stepped on to the pallet to pick up 2 pails and his left foot caught on the shrink wrap. As he was falling he tried to catch himself with his right leg. He pulled the hamstring in his right leg as he fell. Weather conditions were dry.

Employee was in the bathroom and did not lock the door. An employee opened door quickly and door struck left shoulder and pushed employee into the wall.

Employee was doing advanced belt move when the splice on the belt was caught between the roadway and the tail roller. Employee tried to move the belt with his foot when the tension gave way and caught his right foot and sucked his leg into the roller.

GRM was in area 345 working on #2 secondary cooler when EE attempted to use a wedge & pry bar to get the cooler off the studs. When cooler came free, it lifted slightly and upon coming back down on the studs it smashed right index finger. Brought to local ER afternoon of 12/20/15 & was given prescription meds, then seen by ortho specialist on 12/21/15. Had surgery on 12/22/15.

Employee was tying in two headings for blasting and his right knee 'let go' and as he fell to the mine floor he tried to catch himself. Employee injured left wrist and right knee. Employee has had previous incidents with his right knee.

Employee was pulling scoop outside when a chain came loose and struck him in mouth. Employee had to have stitches in his lip.

Over-exertion (Not Elsewhere Classified)

A serviceman injured his right shoulder while cutting a padlock off a truck's tool bin with bolt cutters. Afterwards he swung his arm to test and relieve the pain when his shoulder popped. This caused dizziness and numbness to his arm, forcing him to kneel. After regaining strength, he rose to report it. He was seen locally and in Kayenta where a strain was diagnosed.

Fall from machine

Customer truck driver hauling material for CKJ Trucking LP. Climbed into bed of truck on tar plane of the haul road as he was climbing down from the top of the truck he slipped and fell approx. 5 feet to the pavement below, landing on his right side.

Over-exertion in lifting objects

Employee lifted a drill hammer and felt a ""pop"" in lower stomach. Company received medical reports on 2/9/16 confirming a hernia. Employee has and will, continue to work until he has surgery, which has not yet been scheduled.

Fall from machine

Employee was fueling excavator and when EE turned to dismount EE slipped on tracks and fell to ground catching self with left hand and hurt left shoulder. EE continued to work until June 20th, 2016 where EE had surgery on shoulder.

Struck by falling object

Injured employee was in the process of cleaning material due to pan line gobbing out, when a rock fell off the pan line striking employee's ankle.

Employee pinched his left finger between the belt and a belt winder while repairing the Reclaim belt.

Over-exertion in lifting objects

Employee pick up a draw head to put on the gravel car and feel pain in the left shoulder. This was on 12/17/2015. He continue to work and went to the doctor on 12/28/2015 and he still continue to work until 04/01/2016 when he started to miss work to have surgery on his shoulder.

Employee was stepping back up on a Development Bolter to retrieve a needed wrench. In the process his left foot slipped off the bottom step of the bolter's access causing the employee to roll his left ankle when his foot contacted the ground. Employee was maintaining three points of contact. Employee was sent to the E.R. for an evaluation, x-rays were positive for a fracture.

Employee stopped to pick up a roll of air line. As he bent over he felt something in his back.

Over-exertion in pulling or pushing objects

Employee was moving a piece of scrap pipe across the 2nd floor when he felt pain in his right shoulder.

Struck by falling object

EE (Temporary Employee) was roof bolting when a piece of roof rock approximately 1ft. x 1ft. x 1-4in. fell and struck him on the right hand. He received a laceration and a fracture to his right ring finger. 4 sutures to close laceration and finger splinted.

Over-exertion in lifting objects

The employee was lifting 50 lb bags of lime briquettes into our furnace loading area. Over a two day period the Employee started to have shoulder pains that radiated into his chest. The Employee brought himself to an ER where they determined that he had a rotator cuff issue.

Over-exertion in lifting objects

Cleaning up screen rack, lifted 1"" screen and felt a pull in his left shoulder.

Employee slipped on ice and fell on his knee. He was taken to the ER and released to work. On 12/21/15, employee notified supervisor his knee continued to hurt. He was taken to an orthopedic specialist and taken off work pending the results of an MRI. Currently ONGOING.

An operator was hosing down floors for housekeeping, he noticed a burning sensation to both feet but continued to work. After several hours, the burning sensation progressed to a point of painfulness at which time he removed his boots and realized he had burns to both feet. Holes in his work boots (both) allowed Sodium Hydroxide exposure to his feet.

Employee was opening a drawer in a tool cabinet and a port-a-power ram rolled out of the tool cabinet landing on the top of the employee's right foot. Employee was seen by E.R. x-rays negative but restrictions were issued.

Jan 20, company learned that the employee had suffered a dislocated shoulder resulting in a tear of the labrum that will require surgery.

EE was leaning over a rail by the 5900 ore pocket and was using a long hook to pull rock bolts off grizzly. While attempting to pull on a rock bolt the hook and bolt hung up causing EE to feel pain in his groin area. Later while operating a haul truck EE turned to see behind the truck and he felt pain in his back. EE was taken to a medical provider and was put on restricted duty.

The contract employee was onsite to replace railroad ties. The employee's denim clothing was wet and plastered with mud which contained some lime. The employee developed a rash and was prescribed a pain killer, which he did not take. According to his employer, Railworks, his rash became infected and the Dr. recommended that he not return to work until it was fully healed.

Employee was unloading a pneumatic trailer in a silo. While he was off loading the product, hose became plugged up. He released the tank pressure & began to remove the product hose. When he was swinging a 12 lb. sledge hammer the sledge hammer glanced off the pin hammer, pinching his left index finger between the sledge handle and a steel rub rail on the side of the continuous miner. The EE was preparing a catwalk in order to begin fastening roof decking on a flat roof (total roof area 800 sq ft - 20ft x 40ft). It is unknown how or why the fall occurred, however, the EE was not wearing fall protection equipment as there was no means of tying off. He fell between two rafters that are 2' apart, through the insulation and drywall to the floor below (approx 9 ft).

An employee was changing a motor on Pump 140A, standing position, loosening the base bolts of the pump, after he finished, he turned away from the pump and felt a pop in his right knee. December 14th, the employee was still experiencing pain, at which time an MRI is prescribed, MRI on 15th and on 16th the MRI results indicate a partial tear of the MCL.

Employee states while throwing a garbage bag full of old boots and trash into the back of the pickup he felt pain and a burning sensation on the top of his right shoulder. He started physical therapy on 2/5/16. *Continued to work until surgery on 4/1/2016.

Until 12/13/15, the doctors thought it could be a spider bite or boil under the skin.

Employee was working at the Box Cut area around the Dawes drift mouth. The employee was attempting to move a 6 foot timber out of the walkway and was going to throw it into a crosscut. In the process of picking up the timber and throwing it, the employee twisted his back and felt pain in it.

Employee was using a ladder to assist an employee to adjust a seal. Employee climbed ladder and as he reached to tie off the fall protection lanyard, the ladder moved causing him to slide. The employee fell to the ground bruising the lower back.

Employee was performing clean-up duties using a Belt Tractor. The employee was in the process of backing up when his left foot was caught between the tractor and rock ledge. Employee was working in the VRM on 12/21/15. While walking underneath scaffolding he struck his hard hat on a support and felt his neck jam from the impact. Thought pain would go away and didn't report. Neck pain became worse and started having pain radiating to shoulder. Reported to supervisor on 1/14/16 and was sent to doctor. Follow up visit on 1/25/16 and given modified duty.

Employee was using a hand grinder to cut off the inner race of a bearing that had seized on a fan shaft. The grinder kicked back and cut the left hand that was resting on the fan shaft. The employee's hand was cut just below the thumb knuckle, cutting one tendon and nicking another tendon.

Maintenance man injured right thumb attaching trailer for welder to hitch of pickup truck. Miner taken to ER for initial treatment and returned to work Dec. 11. On Dec. 14, for first available evaluation, orthopedist issued restrictions and confirmed distal tip fracture. Restrictions are being accommodated, no lost time expected, and full recovery expected in 8-weeks.

Injured employee (IE) was trouble-shooting controls for an overhead hoist from a coal mill walkway. IE left the area and fell/stumbled forward while transitioning from one walkway to another. IE's right ear struck the mid-rail of an adjacent guardrail resulting in a 1cm laceration requiring two sutures. This is the first time the IE worked in the area.

The employee was riding in the personnel elevator attached to the 3rd bank silo. The employee complained of lower back pain from the elevator coming to an abrupt stop. The employee did not seek medical attention until 12/14/2015; when he was issued a work restriction (light duty work only).

The EE was released to modified duty work with a restriction of no heavy lifting.

The employee along with two other MZP employees were installing two U-shape (half) rings around a 4 foot diameter X 15 foot long duct pipe. Driver inadvertently pressurized a hose he was disconnecting by bumping open the 1/2"" air supply ball valve. He was sprayed with sulfuric acid from the hose upon disconnecting the air line. A palm sized area on his thigh was burned when the acid passed through an opening in his protective suit.

Lifting a light fixture from shoulder height to ground felt a pop in lower back. Fixture weighed 30 - 40 pounds.

MS FOREMAN AND MS JOURNEYMAN WERE REPLACING PIPING `T's ON THE SIZER IN THE WASH PLANT. THEY SUCCESSFULLY DISASSEMBLED (2) PIPE `T'S. WHILE DISASSEMBLING THE 3RD `T', A COMPRESSION PIPE CONNECTION FAILED. THIS CAUSED THE PIPE AND `T' TO FALL APPROXIMATELY (30'), STRIKING THE JOURNEYMAN ON TOP OF THE LEFT FOOT ABOVE THE STEEL TOE.

Approx 12/31/15 EE was assisting working on material washer, he slipped and fell against the guardrail. This incident was not reported to anyone; he went to his family Dr. On 1/11/16 he went to get on track hoe when he experienced pain on his right side. On 1/13/16 he returned to family doctor and then reported it to HR dept. The safety coordinator was not informed until 1/19/16.

EE was chipping slag and stated he got debris in his left eye. Eye was flushed at the sight and was then evaluated at the hospital. He was given prescription eye medication and returned to work.

EE was walking out of the maintenance shop in the dark & fell on a patch of ice covered from overnight snow. EE landed on EE's left side & arm. EE declined medical attention at the time. EE had a doctor's appointment for a physical on 12/29/2015. During the physical an x-ray found one of EE's ribs was fractured, thus the reason this report wasn't filed at the time of the incident.

While working on the crusher EE fell and struck the side of the crusher. He was wearing a self-retracting fall protection harness but he swung into the side of the crusher resulting in injuries to his face and left arm. He received stitches for his injuries.

Employee was using 385 excavator to pull down material from a 50 ft bench. A single 5-6 tn boulder which had been contained within the face of the material above, toppled off the top while the operator was swinging the excavator. The rock struck the tracks first, then upward into the front end of the cab. Case just became reportable when employee needed to seek physical therapy.

Suspected cause is exposure to mineral fiber while working at the Green Mountain, NC mining operation in the 1970's,1980's and 1990's which mined and processed Olivine. The employee has not missed any work due to his condition.

EE stated that his left knee was bothering him on 12/9/2015 as he walked. He reported this to his supervisor on 12/13/2015. He continued to work until he had surgery on his left knee on 3/1/2016. This is being reported as an occupational illness. Attached MRI noted degenerative chondromalacia and degenerative meniscal deficiency. Additional medical information is attached.

The mine's Safety Department received notification on December 8, 2015 that this retired former employee was granted an award for permanent impairment from noise-induced hearing loss. The incident was caused by the employee's failure to wear the appropriate PPE - hearing protection throughout his career. The employee retired on 12-30-2014.
