THE AREA IS 165 FT LONG BY 45 FT WIDE AND APPROX 20 FT ABOVEROOFLINE. THERE WAS NO EQUIPMENT DAMAGE OR PERSONAL INJURIES.

THE CAUSE IS BELIEVED TO BE FROM PRIMERS FAILING TO DETONATE WHEN IGNITED. THE CAUSE IS STILL UNDER INVESTIGATION.

AT THE BUNKER TAIL SHAFT AREA, MECHANICS WERE GETTING READY TO TAKE SLACK OUT OF ""B"" CHAIN TO PULL SLACK WHEN A WELD THAT HELD THE TENSION ARM IN PLACE BROKE. THE RELEASED TENSION ARM CAME UP THROUGH THE STEPS WHERE THE VICTIM WAS STANDING CAUSING SEVERE LACERATION TO FEMORAL ARTERY IN HIS GROIN AREA.
THE CREW WAS PUTTING BURNER PIPE IN PLACE AND BURNER PIPE FELL 8 FEET STRIKING EMPLOYEE.
THE VICTIM APPARENTLY REMOVED HIMSELF FROM THE OPERATOR'S COMPARTMENT OF THE LONG AIRDOX NO. 3 MBC AND RECEIVED FATAL CRUSHING INJURIES WHEN HEWAS CAUGHT BETWEEN THE MACHINE OPERATOR'S COMPAR TMENT CANOPY AND THE COAL RIB. THE ACCIDENT OCCURRED AS THE MOBILE BRIDGE CONVEYOR WAS BEING SETUP TO MINE IN THE NO. 4 FACE.
EMPLOYEE WAS OPERATING A CRAWLER TRACTOR ON THE #1039 STOCKPILE IN THE MINE PIT. A FLASH FIRE OCCURRED. THE OPERATOR EXPERIENCED EXTENSIVE BURNS AS HE EXITED THE OPERATOR'S CAB. INVESTIGATION CONTINUES. HE WAS HOSPITALIZED FOR 2 1/2 WEEKS IN THE BURN UNIT. ON NOVEMBER 2, 2000, MSHA WAS NOTIFIED THAT HE EXPIRED AT HOME.
