A ROOF FALL 110 FEET LONG, 6 FEET IN HEIGHT AND 20 FEET WIDE OCCURRED ON 7/31/2003 AT SURVEY SPAD NO. 47 TO WITHIN 40 FEET OF SURVEY SPAD NO. 58, TWO BREAKS TO THE LEFT OF THE BELT HEADING AN D APP 1500 FT OUTBY THE FACE.
A ROOF FALL OCCURRED #4 ENTRY ADJACENT TO 4C BELTLINE FROM XCUT 2-3 INCLUDING THE INTERSECTION OF XCUT #2 JUST OUTBY SPAD #6268. L 70' X W 18' X T 10'. THE ROOF WAS BOLTED WITH 6'-10' POINT AN CHOR BOLTS.
POWER TO AN OBSOLETE MANTRIP SAFETY LIGHT SWITCH SHORTED TO THE IRON HOUSING. THE INSULATION ON THE WIRE CAUGHT FIRE AND THE SHORT CIRCUIT BLEW 250 AMP DEFUSES. THIS RESULTED IN THE BRAKE SETT ING ON HOIST. CAGE HAD 14 EMPLOYEES ON IT. THE BAD WIRES WERE ISOLATED AND FUSES WERE REPLACED AND MEN WERE HOISTED TO SURFACE. POWER DISCONNECTED FROM SOURCE AND FROM SWITCHES AND REMOVED.
ROOF FALL WAS DISCOVERED BY EE IN THE #4 ENTRY AT 4 NORTH MAINS. 15' IN BY SS #1892 TO 60' ENTRY. FALL 20' WIDE 8-12' HIGH BY 160' LONG. FALL DOES NOT EFFECT VENTILATION AND WILL NOT BE CLEANED UP.
A ROOF FALL IN THE PILLAR LINE IN THE #1 PILLAR BLOCK. ABOUT 5' TO 6' AT SPAD #1066. THE ROOF IS SHALE THE BOLTS INSTALLED WERE 4' RESIN. THE FALL CAUGHT A REMOTE CONTROLL JOY MINER THAT HAS B EEN REMOVED FROM THE FALL.

THE AREA WA S BOLTED WITH 48"" FULLY GROUTED RESIN BOLTS WITH 6""X6"" PLATES. NO MEN/EQUIP. WERE INVOLVED. VENTILATION WAS NOT INTERUPTED. ALL ACCESSES WERE TIMBERED AND DANGERED OFF.
A FALL OCCURRED IN THE #1 ENTRY OF THE 2 WEST TAILGATE. FALL LOCATION IS APPROX. 40 FT OUTBY SURVEY STATION 26 & 00. FALL IS APPROX. 5' HIGH BY 17' WIDE BY 10' LONG. AREA SUPPORTED WITH 8' RES IN ASSISTED ROOF BOLTS AND 12' CABLE BOLTS. ROCK FELL AROUND THE BOLTS THUS PREVENTING SAFE TRAVEL OUT THE TAILGATE ENTRY. CAUSE OF THIS SMALL FALL WAS BEAM FAILURE. FRICTION CAUSED METHANE TO IGNITE ON THE 48 BUTT EAST SECTION IN THE FACE OF #3 ENTRY APPROXIMATELY 112 FEET FROM THE 58XC. THE BITS APPEARED TO HIT A SULFUR BALL AS THE OPERATOR TRIMMED THE T OP. THE ROOF BOLTERS EXTINGUISHED THE FLAME WITH THE WASH DOWN HOSE AFTER IT BURNED FOR APPROXIMATELY 18 SECONDS.

I FELL FROM THE #5 ENTRY THROUGH THE CROSS CUT AND INTO THE #6 ENTRY ABOUT 10 FEET. FALL MEASURED 64'L X 18' 20'W X 4'THICK. NO PERSONNEL OR EQUIPMENT INVOLVED. AREA WAS TIMBERED OFF AND FALL WILL NOT BE CLEANED UP.
KUHNTOWN ELEVATOR TAKEN OUT OF SERVICE FOR MORE THAN 30 MINUTES DUE TO THE DAILY EXAMINER DETECTING SEVERAL LOOPS OF CABLE CROSSED OVER EACH OTHER IN THE SHEAVE WHEEL GROOVES. INDUSTRIAL ELEVA TOR SERVICE COMP. CORRECTED THIS PROBLEM BEFORE PUT BACK IN SERVICE.

A ROOF FALL OCCURRED BETWEEN #1 AND #2 ENTRIES OF THE SOUTH EAST PANEL AT SURVEY STATION 12 AND 60. THE FALL MEASURED 50' LONG BY 6"" WIDE BY 8' HIGH. ALL APPROACHES TO THIS AREA HAVE BEEN TIMB ERED OFF.
A FIRE WAS DISCOVERED IN THE CLEAN COAL STOCKPILE MSHA INSPECTORS WERE ON SCENE WHEN FIRE WAS DISCOVERED. A PLAN WAS IMPLEMENTED TO EXTINGUISH THE FIRE.
HOISTING UNDER NORMAL CONDITIONS, THE NUMBER FOUR ROPE OF A FOUR ROPE SET ON THE LOADED SOUTH SIDE SKIP BROKE APPRORX 330' FROM THE SURFACE DUMP POINT. THIS BREAK WAS DETECTED BY THE ROPE SLIP INDICATOR AND FORCED THE HOIST INTO AN EMERGENCY STOP. INSPECTIONS OF THE AREA AND EQUIPMENT REVEALED NO ADDITIONAL DAMAGE. AT THIS TIME THE DECISION WAS MADE TO REPLACE ALL FOUR HOIST ROPES
INTAKE BETWEEN #7 AND #8 ENTRY, X CUT #8 ENTRY WAS DROPPED AND FALL CRIBBED OUT #1 UNIT. 30 FT LONG 18FT WIDE, PULLED 5 FT BOLTS.

THE COAL PILE WAS ALLOWED TO ACCUMULATE TO THE POINT THAT THE PILE STARTED TO GET HOT BY SPONT COMBUSTION.
THE ELEVATOR AT THE JUSTICE NO.1 MINE WAS DOWN DUE TO THE DOORS WOULD NOT OPEN (ELECTRICAL)
A SECTION OF THE RIB SLOUGHED ONTO THE EAST RAMP BETWEEN THE 4400 AND 4600 LEVELS. VEHICULAR TRAFFIC WAS BLOCKED UNTIL THE AREA WAS MUCKED OUT & REBOLTED. PEDESTRIAN TRAFFIC WAS NOT IMPEDED NO R WERE OTHER MEANS OF EGRESS.

AT 1:20 PM SUGAR RUN ELEVATOR STOPPED 100' DWON WITH WAYNED CONAWAY ON BOARD. AT 2:40 PM THE ELEVATOR WAS BROUGHT BACK TO THE SURFACE AND EE GOT OFF. CONTACTOR 6B WAS NOT RELEASING THE BRAKE F ULLY AND CAUSED THE 11A OVERWADS TO BURN UP. ELEVATOR REPAIRMAN REPAIRED PROBLEM.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE FACE OF THE #4 ENTRY AT #2 BREAK. SIZE OF FALL WAS ESTIMATED TO BE 25'L X 18-20'W X 7'H. AREA HAD BEEN SUPPORTED WITH 6' TENSION REBAR BOLTS WITH SUP PLEMENTAL SUPPORT CONSISTING OF 10' ROPE BOLTS. NO INJURIES, VENTILATION WAS NOT INTERRUPTED, NO EQUIPMENT CAUGHT. FALL WAS CLEANED UP.
A FIRE WAS DISCOVERED IN THE CLEAN COAL STOCKPILE. THE HOT COAL WAS REMOVED TO EXTINGUISH THE FIRE.
THE BELTS STOPPED AT ABOUT 8:00 P.M. THE SECTION FOREMAN WENT TO INVESTIGATE AND DISCOVERED THE FALL. IT IS 30' LONG, 16' WIDE & 3' TO 4' THICK. THE FALL STOPPED AT THE RIDER SEAM.
THE HOIST MOTOR COUPLING BETWEEN THE MOTOR & PINION SHAFT FAILED (STRIPPED TEETH). THE MINE WAS EVACUATED AT 10:00 P.M. THE HOIST WAS REPAIRED AT 6:45 A.M. THE HOIST WAS TESTED & MINERS RETURN ED UNDERGROUND AT 7:00 A.M.
EE WAS OPERATING A HAUL TRUCK WHEN EE POSSIBLY BLACKED OUT. THE HAUL TRUCK STRUCK ANOTHER TRUCK RESULTING IN DRIVER BEING TRAPPED IN TRUCK FOR 45 MINUTES. ALSO EE WAS UNCONSCIOUS FOR A SHORT PERIOD OF TIME. EE HAS PRIOR HISTORY OF HEART PROBLEMS. MEDICAL EVALUATION NOT COMPLETE AT THIS TIME.
AT ROOF FALL WAS DISCOVERED IN THE NO.5 ENTRY INTAKE OF THE 002-0 MMU THE FALL IS APPROXIMATELY 3,000 FEET OUTBY THE ACTIVE SECTION ABOUT 50' OUTBY SURVEY STATION 4378. THERE WERE NO INJURIE S OR EQUIPMENT INVOLVED. VENTILATION WAS NOT AFFECTED. THE FALL MEASURED APPROXIMATELY 70FT LONG BY 20 FEET WIDE BY 7 FEET HIGH.
A ROOF FALL WAS DISCOVERED IN THE NO.6 ENTRY 30' OUTBY SURVEY STATION NO. 2884 OF 2 RIGHT OFF A NORTH ABOUT 500 FEET OUTBY THE MMU 001-0. THE FALL MEASURED 5' HIGH BY 18' WIDE BY 12' LONG. VEN TILATION WAS NOT AFFECTED. THERE WERE NO INJURIES AND THERE WAS NO EQUIPMENT INVOLVED.
UNINTENTIONAL ROOF FALL #26 CROSSCUT #3 ENTRY INTHE INTERSECTION, ENTRY WAS ADJACENT TO THE TRAVELWAY. 30' LONG BY 30' WIDE 8-10' HIGH. AREA WAS BOLTED WITH 8' TRS BOLTS.
WHILE HAULING OVERBURDEN FROM A 2800 P & H ELECTRIC SHOVEL, A LIEBHERR 240 TON HAUL TRUCK CAUGHT ON FIRE. IT HAD REACHED THE DUMP SITE AND JUST PRIOR TO HOISTING THE BOX THE OPERATOR SAW FLAME S ERUPT FROM UNDER THE CAB AND FRONT DESK AREA. HE SHUT THE UNIT DOWN, INITIATED THE FIRE SUPPRESSION SYSTEM AND EXITED THE TRUCK. MINE RESCUE UNITS & COUNTY FIRE EQUIP EXTINGUISHED THE FIRE
HAD ROCKFALL ON #4 UNIT SUPPLY ROAD XC-#5-#7 6'X20'X150'.
THE SUGAR RUN ELEVATOR STOPPED OPERATIONS WHEN THE DAY SHIFT CREW STARTED UP ON THE ELEVATOR. THE WEIGHT OF THE CREW CAUSED THE ELEVATOR TO PULL THROUGH THE BRAKES AND TRIGGER THE LEVER LIMI T SWITCH. ELEVATOR REPAIRMAN ADJUSTED THE BRAKES.
CONTRIBUTING FACTORS TO THIS INCIDENT WERE SOLELY DUE TO THE HORIZONTAL EXHAUST ON THE OFF-ROAD VEHICLE. THE OFF-ROAD TRUCK APPARENTLY BACKFIRED AND IGNITED A HILLSIDE OF DRY SAGEBRUSH. SINCE THE INCIDENT THE EXHAUST HAS BEEN REDIRECTED TO ESCAPE VERTICALLY UPWARD VS. HORIZONTALLY.
ON 7/21/03, A ROOF FALL WAS DISCOVERED IN THE INTAKE @ SPAD 511. THE FALL MEASURED 20'LONG X 20'WIDE X 8'THICK. 6'RESIN BOLTS WERE TYPE OF SUPPORT IN THIS AREA. THE FALL WAS TO RIDER SEAM AND AREA WAS WET. AREA WAS POSTED AND DANGERED OFF. NON PRODUCING STATUS.

ON 7/21/03, A ROOF FALL WAS DISCOVERED ON #3 BELT LINE AND INTAKE AT SPAD 521. THE FALL MEASURED 100'LENGTH, 20'WIDE, & 10'THICK. 6'RESIN BOLTS & PIZZA POST WERE TYPE OF ROOF SUPPORT USED IN THIS AREA. THE FALL WENT UP TO A RUDDER SEAM AND THE AREA SHOWED SIGNS OF WATER. AREA WAS POSTED & DANGERED OFF. NON PRODUCING STATUS.
DURING A WEEKLY INSPECTION OF THE INTAKE AIR COURSE A ROOF FALL WAS FOUND IN THE #7 ENTRY ADJACENT FROM SURVEY POINT 2243 ON THE 001-0 SECTION. THE FALL MEASURED 6' BY 80' BY 19'. VENTILATION WAS INTERRUPTED BUT STOPPINGS WERE MOVED TO OPENTHE PRIMARY ESCAPEWAY.
ON JULY 19, 2004 AN UNPLANNED ROOF FALL OCCURRED AT THE JUSTICE NO.1 MINE AT #4 BELT, BRK 22. THE FALL MEASURED 20' X 25' X 8' TO 12'.

AT THE TIME THE FIRE STARTED THE AMBIENT AIR TEMPERATURE AT THE WORK AREA WAS APPROXIMATELY 100 DEGREES F. THE PROBABLE FUEL SOURCE WAS HYDRAULIC OIL AND THE LOCATION OF THE FUEL AND IGNITION SOURCE IS YET TO BE DETERMINED. ALL COMPONENTS WILL BE EXAMINED AND TESTED WHERE POSSIBLE. THE FIRE SUPRESSION SYSTEM IS ALSO BEING INVESTIGATED.

WHILE PERFORMING PREVENTIVE MAINTENANCE, A CRACKED COUPLER WAS DISCOVERED BETWEEN MAIN GEARBOX & DRUM SHAFT. THE HOIST WAS SHUT DOWN & MINERS EVACUATED. A NEW COUPLER WAS INSTALLED & THE HOIST TESTED AND RESTORED TO SERVICE AT 3:50 AM, 07/9.
WE EXPERIENCED A LIGHTING STORM THAT CAUSED A POWER OUTAGE. THE HOISTE BREAKER TRIPPED BECAUSE OF THE STORM. THE HOISTEMAN TRIED TO RESET THE BREAKER, BUT IT WOULDN'T BECAUSE IT HAD BECOME OVE R HEATED. ALL MINE PERSONEL WERE EVCUATED VIA THE EMERGENCY SHAFT AND PLANT ELECTRICIAN WERE CALLED TO HANDLE THE BREAKER ISSUE.

A FIRE WAS DISCOVERED IN THE #1 SILO AT APPROX. 11:45 P.M. ON SUNDAY, FULY 6, 2003. THE FIRE APPEARED TO BE APPROX. 4 TO 5 FEET IN DIAMETER AND 3 TO 4 FT IN HEIGHT. THE COAL THAT WAS BURNING W AS EXTINGUISHED BY RUNNING IT THRU THE PREPARATION PLANT. MSHA INVESTIGATED THE INCIDENT ON JULY 8, 2003.

HE TRACED THE SMOLDERING LIGNITE TO THE DUST COLLECTO R & WATERED DOWN THE HOT SPOT & THE FLOOR BENEATH IT. VARIOUS COMMUNICATIONS WERE MADE & THE FIRE DEPT HOSED DOWN THE COLLECTOR FROM 1:30-2:00 PM. CAUSE OF FIRE HAS NOT BEEN DETERMINED. MSHA
METHANE INUNDATION WAS EXPERIENCED ON LONGWALL NO. 16 DUE TO AN EXTREME BUMP AND HOOVING OF THE BOTTOM WHICH CREATED FRACTURES RESULTING IN METHANE BLEEDERS IN THE GOB AREA OF JACK 95. ADDITIO NAL VENTILATION CHANGES WERE MADE TO DILUTE AND CARRY AWAY THE METHANE.
FOUR SHIELD CARS WERE BEING LOADED INTO THE MINE, ONE ELECTRICAL FUSE BLOWN TO THE ELECTRIC MOTOR, WHEN THIS OCCURRED THE ELECTRIC BRAKE WAS INOPERATIVE THEREFORE THE MANUAL BRAKE CAME INTO PL AY. THE CARS STARTED TO GO FASTER DOWN THE SLOPEAND STRUCK A MOTOR PARKED AT THE BOTTOM AND DERAILED THE BRAKECAR. THE HOIST OPR DID GET THE TRIP SLOWED DOWN TO PREVENT DAMAGE.

OUR ELEVATOR AT OUR HART ST. PORTAL BECAME INOPERATIVE AT AROUND 3 PM BECAUSE OF AN ELECTRICAL MALFUNCTION. A SET OF CONTACTS RELEASING THE BRAKING SYSTEM DID NOT OPERATE PROPERLY CAUSING THE BRAKES TO NOT RELEASE. REPAIRS WERE PERFORMED & THE ELEVATOR WAS RETURNED TO SERVICE AROUND 3:40 P.M.

THE FALL WAS DISCOVERED ON PRE-SHIFT EXAM ON 7-2-03. FALL WAS LOCATED IN THE NUMBER 5 HEADING LAST OPEN CROSS CUT OF THE 2 WEST MAINS. FALL WAS 18 FEET WIDE AND 18 FEET LONG WITH UNKNOWN HEIGH T. ROOF HAD BEEN SUPPORTED WITH 5 FOOT TORQUE TENSION BOLTS WITH SUPPLEMENTAL 12 FOOT CABLE BOLTS.
A ROOF FALL WAS FOUND BY THE FOREMAN IN THE EAST MAINS BETWEEN #19 AND XC #20 IN #2 ENTRY. IT MEASURED APPROXIMATELY 30' X 20' X 6'. THE AREA WAS MINED IN MAY 2000 AND WAS SUPPORTED BY TIMBERS AND 6' TENSION REBAR BOLTS. FALL WAS CAUSED BY AN UNDETECTED SLIP IN THE MINE ROOF.
A ROOF FALL OCCURRED AT 2 + 05.3 WAY INTERSECTION #3 ENTRY F-4 OFF 3 NORTH. THE FALL WAS ABOVE ANCHORAGE. THERE WAS NO DAMAGE TO EQUIPMENT. THE FALL WAS 20'WIDE, 14'HIGH & 30'IN LENGTH.
BOOM TRUCK OPERATOR APPARENTLY SWUNG LOAD NEAR POWERLINE. EMPLOYEE LENDING A HAND TO UNHOOK LOAD WAS ELECTROCUTED.
EMPLOYEE WAS IN THE PROCESS OF REPAIRING #3 SHUTTLE CAR CABLE. POWER TO CABLE HAD BEEN TAGGED & LOCKED OUT. HE WAS DISCOVERED LYING ON HIS BACK & NOT BREATHING. CPR WAS ADMINISTERED AND HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS PRONOUNCED DEAD AT APPROX. 8:20 P.M. MSHA/STATE INVESTIGATIONS ARE INCONCLUSIVE AT THIS TIME.
ON JULY 2ND, 2003, EMPLOYEE WAS STRUCK IN THE NECK BY A PROJECTILE WHILE CUTTING COAL WITH A CONTINUOUS MINING MACHINE. PRELIMINARY FINDINGS SUGGEST THE VICIM WAS STRUCK BY A SMALL PIECE OF METAL FROM THE TIP OF A CUTTING BIT.
EMPLOYEE WAS IN THE PROCESS OF MOVING THE SUPPLY FORKS ON A MINI TRACK WHEN A FORK SLIPPED AND CAUGHT THE EMPLOYEES FINGER BETWEEN THE FORK AND THE MOUNTING FRAME.
On the 7-17-03 ee was loading 7' round timbers into a scoop bucket when he felt a pain in groin area. he continued to work until 5-4-04. at that time he was informed by a dr he had a hernia that would require surgery.
HE WAS ON HIS MACHINE AND IT MALFUNCTIONED...THE BLADE CAME DOWN TOO QUICKLY. WHEN HE GOT HIS HAND AWAY FROM THE BLADE HIS LEFT INDEX FINGER GOT TRAPPED BETWEEN THE STONE AND THE HEAD OF THE M ACHINE. THAT TOOK THE TIP OF HIS FINGER OFF.
WORKER DISCOVERED A HERNIA ON HIS LEFT ABDOMEN.
EE WAS CLEANING OUT LIME BUILDUP IN THE HYDRATE FINISHED PRODUCT SCREW CONVEYOR WHEN HIS RIGHT HAND GOT CAUGHT IN THE CONVEYOR RESULTING IN AMPUTATION OF HIS RIGHT HAND ABOVE THE WRIST. APPARE NTLY THE SCREW STARTED UP WHILE HE HAD HIS HAND INSIDE THE SCREW TROUGH. ANOTHER EE HEARD HIS SCREAMING AND CAME TO THE SCENE AND TURNED THE EQUIPMENT OFF.
EE WAS ROOF BOLTING AT THE TIME OF THIS INCIDENT. HE HAD INSTALLED HIS RIB PIN & WAS GETTING READY TO INSTALL CENTER PIN. HE GRABBED HIS STEEL OFF OF THE OPERATORS TRAY, FAST FED HIS DRILL POT DOWN & THE POT OR THE ARM CAUGHT THE END OF HIS STEEL CAUSING THE STEEL TO BE DROVE INTO THE TOP OF HIS FOOT.
INJURED WAS LOADING BELT STRUCTURE WHEN HE MASHED FINGER ON LT. HAND BETWEEN TWO PIECES OF STRUCTURE. (INJURED WAS WEARING GLOVES).
WHILE ATTEMPTING TO REPOSITION A SKIRT RUBBER ON A CONVEYOR WITH A BAR, THE BAR CAUGHT BETWEEN THE BELT & A ROLLER CAUSING THE BAR TO STRIKE HIS KNEE.
EMPLOYEE ALLEGES HE STRAINED HIS BACK PULLING ON BELT STANDS.
MINER WAS CUTTING BELT TO MAKE A SPLICE. WHILE PULLING HANDLE ON BELT KNIFE HAD PAIN IN HIS BACK.
EE WAS CHANGING OUT BARGES. HE CLIMBED A LADDER FROM THE LOADED BARGE TO THE EMPTY BARGE. HE WALKED TO THE BOW END OF THE BARGE, WHERE HE HAD A LOWER BACK SPASM. WE TOOK HIM FROM THE BARGE TO THE DR.
EE AND A CO-WORKER WERE LOADING 1,000 FT OF 48"" CONVEYOR BELT ONTO A SUPPLY CAR ON THE MINE BENCH USING A IT-28 LOADER AND A WIRE ROPE. A LAP OF BELT HAD FALLEN OFF OF THE STACK. AS EE ATTEMPT ED TO PUT THE BELT BACK ON THE CAR HE FELT PAIN IN HIS LOWER ABDOMEN.
WHILE STARTING BOLT USING AN IMPACT WRENCH AND THE BOLT IN HIS HAND, THE BOLT SPUN CUTTING THROUGH GLOVE AND LEFT INDEX FINGER.
AT THE TIME OF THE INCIDENT, EE WAS CLIMBING DOWN THE LADDER OF A CAT 793 OFF HIGHWAY TRUCK. PART WAY DOWN HIS FOOT SLIPPED OFF A LADDER RUNG AND HE BEGAN TO FALL BACKWARDS. WHEN HE GRABBED TH E LADDER WITH HIS LEFT HAND TO ARREST HIS FALL HE FELT A PAIN AND HEARD A POPPING SOUND IN HIS SLEFT SHOULDER.
MOVING MAN BASKET WITH ANOTHER ELECTRICIAN DVER HANDRAILING THE WEIGHT OF THE BASKET SHIFTED AND GOT AWAY FROM US AND SMASHED MY FINGER BETWEEN THE BASKET AND HANDRAILING CAUSING OPEN WOUND ON MIDDLE FINGER AND BROKER BONE.
TRUCK WAS IN THE PIT. AFTER BEING LOADED IT BACKED UP AND SOMEHOW FELL ON ITS PASSENGER SIDE. EE IS EXPERIENCING DISCOMFORT IN HIS KNEE. DOCTOR WROTE HIM OUT OF WORK EFFECTIVE DEC 22 AND IS GO ING TO DO EXPLORATORY SURGERY 0N DEC 31, 2003.

HE CUT WATER OFF BEHIND ROTARY BREAKER, STARTED BACK DOWN STEPS, SLIPPED AND FELL DOWN TWO OR THREE STEPS AND INJURED LOWER BACK AND RIGHT KNEE. HE LATER WENT TO THE DOCTOR ON 9/9/03 WHO STARTED PHYSICAL THERAPY (ONE MONTH). STARTED MISSING ON 9/9/03.
EMPLOYEE WAS CLOSING WINDOW ON BOB CAT AND CUT HIS FINGER ON METAL CAGE OF WINDOW.
INJURED GOT OFF THE FORKLIFT, WENT UP TO THE PALLETIZER TO CHANGE OVER FROM T-3 TO SUPER MASONRY AND NOTICED A BAG STALLED ON DECLINE BELT. SHE WENT TO PUSH DOWN THE BAG AND A HALF EMPTY SUPER BAG WAS LEFT ON THE WALKWAY. SHE WENT TO STEP OVER THE BAG & CAUGHT HE RIGHT FOOT ON THE BAG & FELL ONTO THE METAL GRATING WALKWAY CAUSING LACERATIONS/ABRASIONS TO KNEES, HANDS AND SHOULDER.
EMPLOYEE WAS HOLDING BOLTS IN HOLE OF CUTTING EDGE WITH FINGERS TO IMPACT WRENCH. NUTS WENT THROUGH GLOVE AND CUT FINGERS.
OPERATOR FAILURE TO DEENERGIZE EQUIPMENT BEFORE INSPECTION AND FAILURE TO KEEP FROM HARMS WAY. STRUCK RIGHT SHOULDER AGAINST RETURN ROLLER.
EMPLOYEE HAS BEEN SUFFERING FROM CHRONIC LOWER BACK PAIN DUE TO STRESS CAUSED BY VIBRATION OF THE HEAVY EQUIPMENT HE USUALLY OPERATES (BULLDOZER AND BACKHOE EXCAVATOR).
PERSON WAS HELPING TO REMOVE SECTION OF 6"" WATERLINE. AFTER PIPE WAS UNCHAINED, THE OTHER 2 MEN HOLDING PIPE FET THEIR END GO & THE PERSON FELL WITH PIPE ON SHOULDER. THE FALL DISLOCATED HIS R IGHT SHOULDER.
SUBJECT WAS TRAMMING AN EXCAVATOR THROUGH WOODS ON ACCESS ROAD WHEN IT CONTACTED A HORNETS NEST. THE NEST FELL ON TOP OF THE ENGINE COMPARTMENT. THE HORNETS SWARMED INTO THE OPERATORS CAB. SUB JECT FELL WHEN ATTEMPTING TO EXIT THE CAB TO GET AWAY FROM THE STINGING HORNETS.
EMPLOYEE ATTEMPTED TO STEP OVER A BAGLINE WITH AIR FLOWING THROUGH IT. HE STEPPED ON PART OF BAGLINE, SLIPPED AND FELL.
HE HAD PAIN IN BACK WHILE PICKING UP A PIECE OF WOOD FROM THE BED OF THE TRUCK (RAILROAD TIE AND TRASH OUT OF RAW COAL STOCK PILE).
TWO MEN REPAIRING BRAKE CHAMBER ON MACK DUMP COMPLETED CHAMBER REPLACEMENT. ONE WORKER WAS TO START VEHICLE, BUILD AIR PRESSURE IN SYSTEM AND OTHER WORKER TO ADJUST BRAKE CLEARANCE. WORKER IN TRUCK FAILED TO PUT TRUCK IN NEUTRAL GEAR, RELEASED CLUTCH, RUNNING OVER WORKER UNDER TRUCK.
LOADING COAL WITH MINER PIECE OF DRAW ROCK FELL FROM BETWEEN PINS HITTING HIM IN HEAD & RIGHT SHOULDER.
EMPLOYEE WAS SHOVELING COAL FROM THE RAIL SIDE OF THE MAIN TRACK. IN THE PROCESS OF SHOVELING HE REPORTED GETTING A CATCH IN HIS BACK. HE DIDN'T MISS TIME OR RECEIVE MEDICAL ATTENTION AT THAT TIME. ON OCTOBER 13, 2003, HE CALLED AND SAID HE WAS CONTINUING TO HAVE PAIN FROM THE INJURY. HE RECEIVED MEDICAL ATTENTION ON THAT DAY.
EE WAS DRIVING THE DUMPSTER STATES HE HIT A LARGE BUMP HE DIDNT SEE. THIS CAUSE HIM TO LEAVE THE SEAT AND HIT HIS HEAD ON ROOF OF CAB. SAID HE TOLD HIS FOREMAN EXPLAIN HE PAIN IN HIS NECK AND FOREMAN TOLD HIM TO GO TO THE DR.
EE WAS WORKING ON LOCOMOTIVE AND SLIPPED AND HURT RIGHT SHOULDER. STARTED MISSING WORK 7-31-03.
EE CAUGHT LEFT LITTLE FINGER ON OCURLIED POST AT BOTTOM OF STAIRS. HE BENT HIS FINGER BACK CAUSING A MINER FRACTURE ABOVE CHIP. THIS ACCIDENT WAS REPORTED TO THE CO ON 8-4-03.
EE WAS CHANGING BATTERIES ON A COAL HAULER AT THE TIME OF THIS INCIDENT. HE HAD JUST FINISHED PLUGGING THE DEAD BATTERY UP TO CHARGE AND WAS WALKING BACK TO GET IN HIS HAULER. HE WAS WALKING B ETWEEN THE BATTERY AND THE RIB WHEN HE HEARD THETHE RIB OR SOMETHING MAKE A NOISE. HE STARTED TO MOVE AND LOOK WHEN A ROCK FELL STIRKING HIM ON THE NECKI. GOT OUT OF VEHICLE WHEN HE WAS WALKING BACKTOWARDS VEHICLE WHEN HE WAS STRUCK IN BACK OF HEAD AND KNOCKED UNCONSCIOUS FOR 30-40 MIN.

THE EMPLOYEE WAS IN THE PROCESS OF DRILLING A HOLE FOR A ROOF BOLT AND HAD CHANGED STEEL. HE THEN ACTIVATED THE ROTATION AND THE LEFT HAND WAS TWISTED AGAINST THE DRILL STEEL CAUSING THE INJURY.
WHILE MINING A ROCK CAME DOWN THE SLOPE, HITTING HIM IN THE CHEST.
A ROCK FELL FROM THE TOP, HIT EE IN THE HEAD. HE MISSED TWO DAYS WORK, WENT BACK ON AUGUST 1, 2003 IS NOW FINE.
EMPLOYEE WAS HAULING MATERIAL TO AN ONSITE CONCRETE PLANT WHEN HE CAME UPON A DRILL PARKED IN THE ROADWAY. HE ATTEMPTED TO PASS THE DRILL ON THE LEFT SIDE WHEN HE CAME TOO CLOSE TO THE EDGE OF THE ROADWAY. THE GROUND WAS SOFT CAUSING THE TRUCK TO TURN ON ITS SIDE.
EE WAS UNLOADING SUPPLIES OFF OF THE SCOOP. APPARENTLY THE SCOOP BUCKET WAS SITTING ON SOME CHUNKS OF COAL. HIS FOOT WAS UNDER THE BUCKET AS THE COAL CHUNKS FAILED - THUS RESULTING IN HIS INJU RY.
EE WAS IN OLD WORKS LOOKING FOR SPADS IN LOW COAL, WHICH FORCED HIM TO BEND AT THE KNEES AND BACK OR ""DUCK WALK"" WHEN HE FELT A POP ON HIS LEFT SIDE NEXT TO HIS TAILBONE.
EE HAD COMPLETED INSTALLING AN ALTERNATOR BELT ON A CAT 24H MOTOR GRADER AND WAS MAKING FINAL CHECKS WHEN A ROCK ROLLED OFF A NEARBY SPOILS PILE AND STRUCK HIM INT HE LEGS. HIS LEFT LEG WAS FR ACTURED AND HIS RIGHT LEG SUSTAINED A 3 INCH LACERATION.
EMPLOYEE EXITED THE LEFT SIDE OF THE CAB AND AS HE WALKED AROUND THE CORNER LOST HIS BALANCE AND HAD TO SIDE STEP (SHUFFLE HIS FEET) TO THE LEFT AND GRAB FOR THE RAILING. THE RAILING DIDN'T SU PPORT HIS WEIGHT AS THE WELD WAS BROKEN. HE HUNG BY ONE ARM FOR A SECOND BUT COULDN'T HANG ON. HE FELL TO THE GROUND ON HIS RIGHT SIDE INJURING HIS RIGHT SHOULDER.
THE ALLEGED INJURY WAS TO HAVE OCCURRED WHILE THE EE & HIS HELPER WERE IN THE PROCESS OF INSTALLING TIMBERS & COLLARS. AS THE EE WAS IN THE PROCESS OF SETTING TIMBERS, AN UNDETECTED PIECE OF D RAW ROCK FELL FROM BETWEEN PERMANENT SUPPORTS IMPACTING THE EE ON TOP OF HIS HARD HAT. THE ROCK MEASURED 24"" X 17"" X 0-8"" THICK & THE OVERALL HEIGHT OF THE AREA WAS 8'4"" HIGH & EE'S APPROXIMA

EE WAS WALKING THRU TUNNEL WHEN ALERTED TO SHUT DOWN CONVEYOR. MADE A QUICK PIVOT MOTION TO RT. LEG, FOOT WAS IN A PILE OF GRANUAL. HE STATES THAT HE FELT A POP AND BURNING SENSATION BUT DID NOT PRECEIVE IT AS ANYTHING TO WORRY ABOUT. HE REPORTED IT TO HIS SUPERVISOR, BUT DENIED NEED FOR MEDICAL ATTN. PAIN INCREASED OVER THE WEEK, TRANSPORTED TO CVIC FOR DX OF RT. KNEE STRAIN.
EE WAS RAISING A HINGED SHIELD ON A BROOKEVILLE ULPC-9B MANTRIP IN ORDER TO ACCESS THE BRAKE HEAD COMPARTMENT, WHEN HE FELT A ""PULL"" OR ""STRAIN"" IN HIS LOWER BACK HE WAS RAISING THE SHIELD. HE CONTINUED TO WORK UNTIL 8/1/03.
EE WAS WORKING IN THE SAMPLE PREP ROOM IN THE LAB. SHE WAS WORKING SAMPLES BETWEEN THE SEPERATOR AND THE PULVARIZER. INJURY DUE TO REPETATIVE MOTION OF JOB DUTIES.
EE WAS COMING BACK TO THE JOB SITE EMPTY AND MET A COMPANY WATER TRUCK ASCENDING THE HAULROAD AT CURVE NO. 18. THEY COLLIDED HEAD ON. THE WATER TRUCK DRIVER WAS NOT INJURED.
THE EE INDICATED THAT OVER A PERIOD OF TIME WHILE ENGAGING IN BOLTING UP AND OTHER CONSTRUCTION ACTIVITIES, THAT HE BEGAN TO FEEL PAIN IN HIS LOW BACK THAT GOT WORSE OVER TIME. WHEN ASKED HE COULD NOT IDENTIFY ANY PARTICULAR TIME OR EVENT WHEN THE PAIN STARTED.
Employee was dragging a plastic driscol pipe up the ramp and lost his footing. While falling forward he twisted his back. Sought medical treatment on 7/29 and was released to full duty. Has been having intermittant back pain. Worked on 12/2 and felt tingling in is leg on 12/3. Went to the Doctor on 12/3 and was taken off work until further evaluation. First shift missed 12/3.
WHILE DIGGING IN AN AREA WITH A LARGE""TOE"", A BOULDER FELL FROM ATOP THE TOE ONTO THE CORNER OF THE FEL BUCKET. THE FALL WAS NOT FROM A GREAT HEIGHT BUT THE SUDDENESS OF IT AND THE MASS OF THE BOULDER CAUSED THE FEL TO BE JARRED AND THE EE WAS JARRED ALONG WITH IT. THE EE HAD SOME PAIN AT FIRST BUT DID NOT MENTION IT UNTIL LATER IN THE DAY WHEN IT GOT WORSE.
THE EE BENT DOWN TO UNLOCK SWITCH GEAR: LIFTED UP ON SWITCH TO RELEASE LOCK, AND FELT SHARP PAIN IN MIDDLE LOWER BACK.
HE SAID HE WAS MOVING BIG ROCKS AND PUTTING THEM ON PALLETS. INJURY DATE WAS 7/28/03. HE WENT TO THE DOCTOR ON AUG. 6, 2003.

WHILE IN THE PROCESS OF ROCK DUSTING EE PICKED UP A BAG OF DUST WEIGHING 50 LBS. HE STATED WHILE LIFTING THE BAG OF DUST HE EXPERIENCED A SHARP PAIN IN HIS LOWER BACK THAT RADIATED INTO HIS GR OIN AREA AND RIGHT LEG.
HE WAS SITTING IN SEAT OPERATING MACHINE. STARTED SWINGING MACHINE AROUND AND DROPPED BUCKET ON THE GROUND CAUSING A JOLT.
EE WAS REMOVING AN OVERHEAD CHAIN FALL (HOIST) FROM THE RIGGING, STANDING ON 6' STEP LADDER TO REACH THE UNIT HANGING OVERHEAD. THE CHAINFALL SAFETY LATCH CAUGHT ON THE RIGGING CAUSING THE EE TO PULL ON THE UNIT. THE UNIT BACAME LOOSE CAUSING THE EE TO LOSE HIS BALANCE CAUSING HIM TO FALL APPROX. 4' FROM THE 6' STEP LADDER LANDING ON HIS RIGHT ARM.

WHILE LOADING A FOAM FILLED SHUTTLE CAR TIRE ON A A FLAT CAR EE STEPPED IN A HOLE ON THE CAR. THE TIRE SLID ON OFF THE SCOOP BUCKET FACING THEBACK OF HIS RIGHT LEG INTO THE SOLID PART OF
INJURED PARTY WAS HELPING CHANGE A STARGATE SHAFT IN TILE PREPARATION PLANT. WHEN THE HOUSING WAS TAKEN OFF THE ONE END IT CAUSED THE END HE WAS HOLDING TO RAISE UP SUDDENLY SMASHING HIS RING FINGER ON HIS RIGHT HAND.
EE WAS DESCENDING A LADDER YESTERDAY AND STEPPED TO LOWER RUNG WITH RIGHT FOOT WHILE LEFT FOOT TWISTED. EE FELT A SHARP PAIN AND HEARD A POP.
UNKNOWN- EE DID NOT REPORT AN ACCIDENT. HE REPORTED TO HIS SUPERVISOR HE WOULD BE OFF ON A PRIOR INJURY FROM ANOTHER COMPANY. OUR FIRST NOTIFICATION OF AN INJURY WAS A COMP. FORM RECEIVED ON 9/30/03 INDICATING A BACK INJURY.
EMPLOYEE WENT TO HOSPITAL ON 7/26/03 AND STAYED OVERNIGHT FOR OBSERVATION. WHILE DRIVING A COMPANY CRANE IT ROLLED OVER, PARTIALLY PINNING EMPLOYEE AND EMPLOYEE RECEIVED MULTIPLE INJURIES. EMP LOYEE WAS TAKEN OFF WORK BY HOSPITAL PHYSICIAN.
EE WAS REPAIRING A WOOD PALLET AT THE PACKING HOUSE. HE WAS USING A PIECE OF WOOD AS A LEVEL TO SEPARATE A DAMAGED PIECE OF WOOD FROM THE PALLET. SUDDENLY THE DAMAGED PIECE OF WOOD BROKE AND H IT HIS NOSE.
EE WAS PULLING CABLE AT THE END OF SHIFT SATURDAY WHEN HIS FOOT GOT STUCK IN THE MUD AND HE TWISTED HIS KNEE. HE DID NOT REPORT UNTIL MONDAY.
On July 15th 2003 the employee was cleaning up around the shop area. July 16th the employee came to work and complained of back soreness. On Aug 20 the employee stated he could not work anymore because of his back pain. This became a lost time injury on Aug 20, 2003.
INJURED PARTY WAS HELPING WORK ON A CONTINUOUS MINER BACK ON THE 7/25/03 WHEN HE SAID HE GOT A PIECE OF ROCK ON HIS RIGHT KNEE. THE ROCK FELL OUT BETWEEN ROOF BOLTS HE SAID. HE JUST WENT TO TH E DOCTOR THE 8/27/03 FOR THE FIRST TIME & HAS NOT MISSED ANY TIME UP TO THIS DATE OF 8/27/03.
INJURED WAS HELPING CHANGE OUT A TRAM MOTOR ON A MINER. HE WAS SITTING IN THE SCOOP BUCKET WHICH THEY USED TO CHANGE THE MOTOR WITH WHEN A PIECE OF DRAW ROCK FELL OUT BETWEEN BOLTS, MEASURING 17' LONG, 3' WIDE & 1/4"" THICK ON ONE END & 7"" ON THE OTHER END. THE ROCK STRUCK HIS SHOULDER & PUSHED HIM OVER HURTING HIS BACK.

OVER THE NEXT FEW DAYS, HIS BACK BECAME STIFF AND SORE. HE WORKED 7/26 AND 7/28 - WENT TO DR ON 7/28 AND THEN WAS OFF.

HE HIT HIS HEAD ON A ROOF BOLT, INJUR ING HIS NECK AND BACK. AFTER HITTING HIS HEAD HE FELL TO THE MINE FLOOR ON HIS REAR CAUSING PAIN TO HIS HIPS AND LOWER BACK.
WHILE DOING REPAIR WORK ON HIGHWALL MINER, EE SLIPPED AND FELL HURTING HIS LEFT HAND.
PARKING BRAKE ON GREASE TRUCK FAILED TO HOLD WHILE GREASER WAS SERVICING DOZER. IT TOOK OFF ROLLING BACKWARD TOWARDS FUEL TRUCK; GREASER JUMPED OFF OF DOZER TO STOP IT, HE HURT HIS BACK.
SPLICED CONV.BELT, AFTER COMPLETING SPLICE EE STARTED BELT & THOUGHT IT WAS RUBBING SOMEWHERE SO HE WAS LOOKING TO SEE WHERE WHEN A ROCK FELL OFF & LANDED ON HIS FOOT. EE BROKE A BONE IN HIS FOOT, EE HAD STEEL TOES ON.
EE WAS WASHING HIS BOOTS, HE DIDN'T REALIZE HIS RIGHT BOOT HAD A HOLE ALLOWING THE HOT WASH WATER TO SEEP ONTO HIS FOOT CAUSING A BURN.
EE WAS CLIMBING LADDER TO DRILL WHEN SLIPPED AND FELL BACKWARDS OFF LADDER.
CAUGHT FINGER BETWEEN PIPE ON CRANE AND STANDARD ON COG TRUCK.

EE STATES SHE EXPERIENCED A PULLING PAIN IN HER BACK AS SHE ATTEMPTED TO CLOSE THE FREIGHT ELEVATOR DOOR ON THE THIRD FLOOR OF THE MILL BUILDING.
EE WAS TRYING TO FREE THE CROW'S FOOT HUNG ON A HOG PANEL WHEN THE CROW'S FOOT ROLLED OVER AND PINNED EE S RIGHT ARM.
EE WAS TRYING TO LIFT A BATTERY LID ON A SCOOPOR RAMCAR BATTERY. STRAINED HIS SHOULDER. HINGES WERE GOBBED OUT. DID NOT KNOW OF THE INJURY UNTIL THE 29TH. COULD NOT REACH HIM TO FIND OUT ANYT HING MORE.
WHILE WIDENING THE ROADWAY, SPOIL APPARENTLY DISLODGED & SLID, RESULTING IN A BREACH OF THE COUPLING OF THE NATURAL GAS TRANSMISSION LINE, WHICH APPARENTLY IGNITED FROM THE BLOWER OF THE DOZER , INFLICTING 3RD DEGREE BURNS TO THE OPERATOR, & FRACTURING LOWER LEFT LEG.
BOLT MAN WAS IN #4 FACE BOLTING WHEN HE WAS BENDING A BOLT BACK TO SET ON THE TOP OF THE POT. HIS LOWER BACK STARTED HURTING. HE WORKED THE FOLLOWING TWO DAYS UNTIL IT HURT TOO BAD.
EMPLOYEE WAS MOVING A SMALL PIECE OF ROCK BY HAND FROM IN FRONT OF A MOBILE ROOF SUPPORT AND INADVERTENTLY ALLOWED HIS LEFT HAND TO GET CAUGHT BETWEEN THE ROCK AND THE FRAME OF THE MRS. *EMPLO YEE DIDN'T START MISSING WORK UNTIL 7/28/03.
EE WAS GETTING A SAMPLE OF #8 GRAVEL FROM THE STOCK PILE AT PLANT IN A THREE GAL BUCKET AFTER PUTTING APPROX 15 OB OF GRAVEL IN THE BUCKET HE LEANED OVER TOOK A COUPLE OF STEPS WHEN HE FELT A SHARP PAIN IN HIS BACK.
WAS MAKING A BELT SPLICE AT #3 HEAD DRIVE, EE PICKED UP A BELT SPLICE BOARD AND GOT A SEVERE PAIN IN LOWER BACK.
EE WAS CLIMBING ON BLAST HOLE DRILL FRAME AND SLIPPED CATCHING HIMSELF WITH HIS LEFT ARM. INITIALLY DID NOT SEEK MEDICAL ATTENTION BUT WHEN SHOULDER PAIN DID NOT IMPROVE WAS SEEN BY A DR ON 7- 29-03 AND RELEASED BACK TO RESTRICTED DUTY PENDIING MRI.
EE WAS ADJUSTING THE GUARD ON THE TIP OF THE VACTOR HOSE WHILE EQUIPMENT WAS OPERATING. HIS ARM WAS SUCKED INTO THE HOSE.
EE was opening track door going over to examine seals when a piece of rock fell from top. he moved quickly to dodge rock and hit head on door facing door open approx. 60"" high note: ee did not miss work until 10-21-03. he stated that he was off due to an old injury and it was not discovered until 5-3-04. that it was this accident.
THE EE WAS HAND LEVELING THE FLOOR OF A SHALLOW TRENCH FOR A SEWER LEECH FIELD. A SIDE WALL CAVE AND BURIED THE MANS LEGS UP TO HIS KNEES. HIS LEFT KNEE TWISTED CAUSING THE INJURY. THE MAN WAS FLOWN TO THE DILLINGHAM HOSPITAL FOR X-RAYS.
EE WAS CHANGING THE SCREENS TO A SCREEN DECK WITH AN IMPACT WRENCH. HE WAS SWEATING & HE WIPED HIS BROW. HE FELT SOMETHING ON HIS EYELASH, AND WHEN HE WIPED HIS LASHES, HE FELT SOMETHING IN HIS EYE.

MOVING A ROCK OUT OF THE WAY, IT ROLLED OVER HISHAND & INJURED HIS WRIST.
EE WAS IN THE PROCESS OF MOVING MINER CABLE OVER WHEN A PIECE OF DRAW ROCK FELL FROM THE ROOF AND STRUCK THE EE IN HIS LOWER BACK. FIRST DAY AWAY FROM WORK WAS 7-31-03.
EE CLAIMS TO HAVE MISSED A STEP ON A 3 STEP PLATFORM, FELL AND TWISTED HIS LEFT ANKLE.
EE WAS USING A CUTTING TORCH TO CUT OFF BOLTS OF A CONNECTING CLAMP ON A 10"" WATER LINE APPROX. 40' LONG. THE BOLTS BROKE LOOSE ALLOWING THE PIPE TO DROP ABOUT 2', STRIKING HIS LEFT SHOULDER & ANKLE RESULTING IN A FRACTURE TO HIS ANKLE.
EMPLOYEE WAS WALKING TO A WORK AREA IN THE PLANT. HE WALKED UNDER A SUPPORT BEAM FOR A MOTOR AND RAISED UP BEFORE HE WAS CLEAR OF THE BEAM, STRIKING HIS HEAD. HE REPORTED STIFFNESS AND PAIN IN HIS NECK AND SHOULDERS.
PERSON PARKED MANTRIP (HUMMER) AND WALKED UP TO AREA FOR SOME REASON. HE WAS STRUCK BY CRIB BLOCKS WHEN CRIB TOPPLED OVER AND STRUCK HIM. AREA WAS ON HUMMER HAULROAD (F-MAINS) BETWEEN #70 & #7 1 BK ON LEFT SIDE FACING INBY.
EE WAS HAMMERING ON A CONNECTING LINK ON A CONVEYOR CHAIN FOR CO-WORKER. HE STATED HE TRANSFERRED HIS WEIGHT TO THE RIGHT KNEE AND FELT SOME GRINDING IN HIS KNEE. EE IS SCHEDULED TO HAVE SURGE RY ON AUG 8.
EE WAS ON HIS KNEES WORKING ON A CONTINUOUS MINER, WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF BOLTS HITTING HIM ON THE BACK SIDE OF LEFT LEG, BELOW KNEE. ROCK SIZE WAS 30"" LONG 16"" WIDE 1"" TO 2"" THICK. HEIGHT WAS 7'. 1ST DAY FROM WORK WAS ON 7-30-03.
INJURED WAS BOLTING THE MINE ROOF WITH 6' ROOF BOLTS AND AS HE STARTED TO ROTATE THE BOLT IN ORDER TO GET A BEND IN THE DRILLED HOLE, THE BOLT CAME OUT OF THE CHUCK AND STRUCK HIM ON THE THUMB RESULTING.
WHILE RIDING IN THE SKIP GOING DOWN THE SHAFT, EE CAUGHT AN ARTICLE OF CLOTHING AND HAD HIS HAND PULLED IN BETWEEN THE BUCKET AND THE SHAFT. THIS RESULTED IN INJURING HIS RIGHT HAND.
THROUGHOUT THE COURSE OF EMPLOYEE'S SHIFT, HE WAS INADVERTANTLY EXPOSED TO WELDING & CUTTING PROCESSES THAT WERE TAKING PLACE AROUND HIM. THIS EXPOSURE RESULTED IN WELDING FLASH IN BOTH EYES. HE WAS TREATED THE FOLLOWING DAY.
WHILE LOADING DRILL STEEL WITH BOOM TRUCK, STEADYING AND BALANCING 1 OF 3 STEELS UNDER THE CONTROL PANEL WITH THE LEFT HAND, LOWERING THE BOOM WITH THE RIGHT HAND, STEEL SMASHED LEFT THUMB BETWEEN CONTROL PANEL AND DRILL STEEL.
EE WAS BRUSHING/CLEARING WITH EXCAVATOR WHEN TREE FELL THROUGH THE MACHINE DOOR AND LANDED ON HIS RIGHT FOOT, PINNING FOOT. RIGHT FOOT WAS CUT ON BOTTOM AND POSSIBLE FRACTURED BONES IN THE FOO T.
INJURED STATED THAT HE WAS CHECKING THE OIL IN THE RD800 GREASE TRUCK AND AS HE WAS LIFTING THE HOOD HE FELT SOMETHING PULL IN HIS LOWER BACK. INJURED HAD NO LOST TIME UNTIL 7-28-03.
EE WAS DESCENDING A 12% GRADE IN THE EMPTY HAUL TRUCK AND FAILED TO MAINTAIN CONTROL AT A SLOWER SPEED. THUS UNABLE TO NEGOTIATE A CURVE IN THE HAUL ROAD AND DRIVING OVER THE EDGE OF THE ROAD AND WRECKING THE TRUCK.
EE WAS TAKING THE BUCKET LOOSE FROM THE MACHINE AND PUTTING THE FORKS ON THE SKID STEER, IN THE PROCESS OF DISCONNECTING THE BUCKET THE BUCKET FELL AND HIT HIS LEFT KNEE AND SHIN BONE.
DID NOT START LOSING TIME UNTIL 9/2/03. WALKING DOWN PANLINE, STEPPED IN HOLE AND TWISTED RIGHT KNEE.
EE WAS MOPPING FLOORS IN OFFICE. SHE FELL ON WET FLOOR LANDING ON HIP & HITTING HER HEAD ON THE REFRIGERATOR.
EMPLOYEE WAS PUSHING REFUSE WITH A D10R DOZER AND WAS HAVING PAIN IN HIS BACK. THE PAIN GOT SO SEVERE HE HAD TO STOP OPERATING THE DOZER. HE SOUGHT MEDICAL ATTENTION. THE EE DID NOT INDICATE T HAT THIS INJURY WAS JOB RELATED UNTIL 8-4-03.
REMOVING GUARD AT FOOT WHEEL A-2 BELT. AREA HAS LIMITED WORKING SPACE. EE WAS IN AWKWARD POSITION WHEN REMOVING GUARD WHICH IS HEAVY. TWISTED BACK. GENERAL AREA HAD A LOT OF HEAVY WET STONE AN D DIRT FROM PREVIOUS HARD RAIN.
EE WAS REMOVING FILL PLUG FROM DIFFERENTIAL OF #05-30 HAUL TRUCK. AS PLUG WAS REMOVED, PRESSURE BLEW HOT OIL ONTO EE'S FACE, NECK CHEST AREA, RIGHT ARM & HAND, & LEFT FOREARM. EE REC'D 2ND & 3 RD DEGREE BURNS TO ABOVE MENTIONED AREAS (APPROX 15% OF BODY). EE WAS TRANSFERRED BY AMBULANCE TO PARKLAND HOSPITAL IN FARMINGTON, WAS LATER TRANSFERRED TO ST JOHNS HOSPITAL IN ST LOUIS.
EMPLOYEE WAS RIDING IN THE BACK SEAT OF A 12-MAN OPEN MANTRIP. AS THE MANTRIP WENT THROUGHT THE AIR DOORS, THE EMPLOYEE RAISED UP ON HIS KNEES TO CLOSE THE DOORS. WHILE HE WAS CLOSING THE DOOR S HE HIT HIS HEAD ON THE AIR-DOOR SWITCH CAUSING A LACERATION TO HIS UPPER LIP AND A FRACTURE TO HIS NOSE AND JAW.
WHILE DRILLING HOLES IN TRUCK BED FOR A COMPARTMENT, DRILL BIT SLIPPED AND SLAMMED DOWN ON TOP OF HIS HAND.

EMPLOYEE WAS PUTTING BOLTER SUPPIES ONTO THE BOLTER WHEN HE EXPERIENCED PAIN IN LOWER BACK.
EE WAS BENDING OVER TO ADJUST WATER VALVE ON #2 COLUMN, AS HE PULLED ON THE VALVE HE FELT A STRAIN IN HIS BACK. LOST TIME DID NOT START UNTIL 07-29-03
WHILE STANDING ON THE RIGHT FRONT TIRE OF THE REAR TANDEN ON A MOTOR GRADER, THE EE HAD JUST FINISHED CLEANING THE INSIDE OF DOOR GLASS, WITH THE DOOR SWUNG OPEN. AS HE STEPPED BETWEEN THE TIR E AND THE CAB, TO ENTER THE CAB, THE DOOR WHICH HE WAS HOLDING ONTO SWUNG FREE (THE HOLD OPEN LATCH ON THE DOOR WAS NOT LOCKED IN). THE EE FELL TO THE GROUND LANDING ON HIS LEFT HIP, FRACTURI
EE PARKED A FORD F250 PICKUP IN ORDER TO GET TOOLS OUT OF ANOTHER VEHICLE. HE LEFT THE TRUCK RUNNING. A SHORT TIME LATER HE NOTICED THE TRUCK ROLLING BACKWARDS & RAN AFTER IT TO STOP IT. WHEN HE JUMPED IN, HE HIT HIS BACK ON THE BACK OF CAB AT THE DOOR OPENING. HE DROVE THE TRUCK TO THE SHOP & WENT TO HIS FAMILY DOCTOR AFTER THE SHIFT. MAINTENANCE FOUND A BROKEN CLIP THAT HOLDS THE
EE WAS CHANGING BITS IN MINER AND BUMPED ELBOW AGAINST MINER BIT - BRUISED RIGHT ELBOW.
THE EE WAS LIFTING BROKEN CEMENT BAGS OUT OF THE HOPPER THAT IS USED FOR WASTE CEMENT, AND PULLED A MUSCLE IN LEFT SIDE OF NECK.
EE HAD CHANGED THE SCRUBBER FILTER AND HAD IT ON THE END OF HIS SCREWDRIVER. THE FILTER SLIPPED OFF THE SCREWDRIVER CAUSING HIM TO JERK IT BACK TOWARD HIMSELF. HE POKED HIMSELF IN THE EYE WITH THE SCREWDRIVER. THIS RESULTED IN AN ABRASION TO THE EYEBALL AND A LACERATED EYELID.
EMPLOYEE STATED THAT AS HE DROPPED THE HEAD OF THE DRILL MAST DOWN THE WRENCH FELL OUT OF THE DRILL POT. IN REACTION HE REACHED FOR THE FALLING WRENCH AND WHILE DOING SO THE FIVE FOOT DRILL ST EEL FELL FROM THE HOLE HE HAD DRILLED AND HIT HIS LEFT THUMB.
TAKING DREDGE PUMP APART, THE PUMP SHELL BUMPED ANOTHER PART CAUSING IT TO PINCH HIS RIGHT ANKLE.
DRIVER WAS EXITING HIS 15C/Y DUMP TRUCK FOR INSPECTION. DRIVER DID NOT HAVE SECURE FOOTING. THE DRIVER'S FOOT SLIPPED FROM THE STEP & CAUSED HIMTO FALL TO GROUND. THE DRIVER RECEIVED MINOR BRUISES & SPRAINED BACK

WAS HITTING ON DISCHARGE DUST COLLECTOR PIPE USING A SLEDGEHAMMER. THIS WAS NOT REPORTED DUE TO MEDICAL TREATMENT DID NOT OCCUR UNTIL 8/28/03.
EMPLOYEE WAS INVOLVED IN AN ACCIDENT IN WHICH HE DROVE THE ROAD TRAIN INTO A HAUL ROAD BERM. HE SUFFERED A BACK STRAIN AND VERTABRAE FRACTURE.
WHILE INSPECTING THE SPIRALS OF THE WASHER MACHINE THE SPIRAL WAS JAMMED WITH SAND, HE PULLED THE WATER HOSE TO FLUSH IT WHEN HIS FEET BECAME TRAPPED IN THE WATER HOSE. HE LOST HIS BALANCE AND WHILE FALLING WAS ABLE TO GRAB A SIDE LADDER. HIS LEFT FOOT GOT CAUGHT BETWEEN THE LADDER AND THE SPIRAL AND SPRAINED HIS LEFT KNEE,LEG AND INJURED HIS LOWER BACK AND HE HIT HIS LEFT ARM. HE
EE STATES HE WAS CLEANING THE GUARD SHACK, SWEEPING THE FLOOR. HE HAD TO MOVE A SMALL COOLER TO SWEEP UNDER IT, HE STEPPED ON THE FLOOR & IT GAVE WAY W/HIM. HIS RT. FOOT WENT THROUGH THE FLOOR WHILE HIS LEFT FOOT/LEG WENT UNDERNEATH HIM.
EE AND CO-WORKER WERE ATTEMPTING TO OPEN A DAMAGED EQUIPMENT DOOR ON THE SOUTH MAINS HAULAGE. THE DOOR WAS DRAGGING ON THE TOP OF THE RAIL & AS THEY WERE PUSHING IT THE DOOR WENT FREE, HE STUM BLED AND HURT HIS FOOT.
EE WAS WORKING IN THE E-10 SECTION PERFORMING HIS NORMAL WORK DUTIES OF INSTALLING 16' ROPE BOLTS AND FELT PAIN IN HIS RIGHT SIDE GROIN. EE BEGAN MISSING WORK ON 7-29-03.
EMPLOYEE WAS OPERATING LOADER AND HIT A POTHOLE AND WAS JARRED IN SEAT, TWISTING BACK. EMPLOYEE SOUGHT MEDICAL ATTENTION FOR THIS 9/16/03. NO INJURY WAS EVIDENT. EMPLOYEE WAS OUT FOR ELBOW SUR GERY AND DECIDED TO GET HIS BACK RE-EXAMINED AND THIS DIAGNOSIS WAS MADE AND THEY ARE NOW TREATING HIM FOR THIS BACK AILMENT.
EE STATED PIECE OF ROCK FELL BETWEEN SHIELD AND STRUCK HIM ON RT LEG.

WHILE BOLTING IN #2 FACE, EE WENT BACK TO MIDDLEOF MACHINE TO DROP ATRS TO ADVANCE MACHINE FORWARD; A PIECE OF DRAW ROCK FELL OUT FROM BETWEEN ROOF BOLTS ALREADY INSTALLED. THE PIN MEN WERE G ETTING READY TO INSTALL THEIR THIRD ROW OF BOLTS.
EMPLOYEE WAS HELPING HANG ROOF BOLTER CABLE WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER. HE DIDN'T START MISSING WORK UNTIL 7/28/03.
EE WAS HELPING LOAD A TIRE INTO THE SERVICE TRUCK. LATER IN THE SHIFT EE STATED HIS BACK WAS HURTING HIM. EE COMPLETED HIS SHIFT AND TRANSPORTEDHIMSELF TO THE HOSPITAL. HE WAS DIAGNOSED WITH PULLED MUSCLES IN HIS BACK.
WALKING FROM THE TRACK ENTRY TO THE BELT ENTRY AND STEPPED ON A CEMENT BLOCK. THE BLOCK MOVED AND INJURED TWISTED HIS RIGHT KNEE.
VOLATILE CLEANERS WERE USED TO CLEAN GREASE IN THE VEHICLE SWING SHAFT BORE WHEN IT WAS DISCOVERED THAT MODIFICATIONS HAD TO BE MADE TO THE LIP TO PLACE THE RETAINER. TARP WAS PLACED IN THE BO RE TO PREVENT DEBRIS FROM GRINDING ENTERING. IT TRAPPED GASES FROM THE EVAPORATING CLEANING FLUIDS WHICH WERE IGNITED BY THE GRINDER.
EE WAS JARRED WHEN THE TRUCK HE WAS DRIVING REARED UP & THEN CAME DOWN SUDDENLY

COAL RIB SLOUGHED OFF AFTER WATER DUKE JACK WAS RELEASED AND RECOVERED. MATERIAL ROLLED AGAINST 4'X 8' PLYWOOD CAUSING PLYWOOD & MATERIAL TO ROLL OVER & STRIKE EE ON RT. FOOT AND LOWER LEG.
EE WAS PUTTING ON A PAIR OF GLOVES. APPARENTLY A SMALL SPIDER (POSSIBLY A BROWN RECLUSE SPIDER)WAS IN THE FINGER OF ONE OF THE GLOVES. THE EE WAS BITTEN ON THE LEFT RING FINGER AND AGAIN ON HIS LEFT FOREARM AS HE WAS QUICKLY REMOVING THE GLOVE.

THE DRIVER OF A ROCK TRUCK FELL ASLEEP AND RAN AHEAD ON INTO ANOTHER ROCK TRUCK.

THE OPERATOR WAS SHIFTING THE MACHINE FROM SWINGTO TRAVEL WITH A FOOT PEDAL, HIS BACK POPPED RESULTING IN SEVERE PAIN IN HIS LOWER BACK.
MEMBER WAS ROOFBOLTING WHEN TOP STARTED FALLING AROUND ATRS OF ROOFBOLTER. WHEN HE MOVED BACK TO GET CLEAR OF ROCK HE STEPPED ON A ROCK AND SPRAINED HIS RIGHT KNEE.
PULLING DE-MISTER OUT OF MINES.
EE WAS IN THE PROCESS OF DUMPING 260 GRADE SODA ASH INTO A DISSOLVING POT. WHEN THE PRODUCT HIT THE WATER IT ERUPTED AND BURNED BOTH OF THE EE'S LEGS.
THE EMPLOYEE WAS DRILLING A HOLE AND A ROCK APPROXIMATELY 14 IN ROUND & 1 1/2 IN THICK FELL FROM THE TOP AND STRUCK LEFT FOOT.
WHILE INSTALLING FLYBOARD HE WAS HOLDING BOTH STEELS AND MOVING POT BACK AND FORTH WHEN THE POT HIT A ROCK CAUSING THE TWO DRILL STEELS TO PINCH HIS FINGERS.
GREASING EQUIPMENT WHEN HE TRIGGER HIGH PRESSURE HOSE FROM CLOSE RANGE STRIKING HIMSELF IN THE EYE WITH A STREAM OF GREASE.
INJURED STEPPED ON A ROCK BETWEEN THE TRACK RAILS AND FELL STRIKING HIS RIGHT HIP ON THE RAIL.
EMPLOYEE ALLEGEDLY BENT OVER TO PICK UP A CLAYBALL. HIS LEFT LEG SLID OFF THE PLATFORM AND HE GRABBED THE RAILING WITH HIS LEFT ARM AND TWISTED HIS SHOULDER. HE DID NOT GO TO A DOCTOR UNTIL 7- 22-03.
WORKING ON EQUIPMENT AND A HYDRAULIC JACK SLIPPED PINNING HIS THUMB BETWEEN 2 PIECES OF METAL.
EMPLOYEE WAS CUTTING A BUSHING OUT OF A LIFT ARM PIT, WIND BLEW FLAME THROUGH BORE AND BURNT RIGHT SIDE OF HIS FACE AND RIGHT ARM.
THE INJURED WAS HELPING EE TO WELD A PLATE TO THE FRAME AT THE SCREEN HOUSE. HE STATED THAT HE WAS WEARING SAFETY GLASSES WITH DARK LENSE.
THE EE WAS LOOKING FOR THE SOURCE OF SMOKE AFTER WELDING OPERATIONS ON A 2800 SHOVEL. HE FELT HEAT ON A MANDOOR AND WHEN HE OPENED THE DOOR FLAMES AND SHOT OUT AND BURNT HIS ARMS AND FACE.
EE WAS LIFTING BLOCKS WHILE BUILDING A PERMANENTSTOPPING WHEN HE FELT PAIN IN HIS BACK.
EE PREPARING TO FILL IN WASHED OUT AREA USING A BULLDOZER. EE WAS WALKING THROUGH THE ROUGH TERRAIN OF THE WASHED OUT AREA WHEN HIS FOOT SLIPPED INTO A WASHOUT INJURING HIS ANKLE.
EE WAS REBUILDING A CARDOX TUBE THAT HAD BEEN INSIDE AN ACTIVE PREHEATER. ONE END OF THE TUBE WAS NORMAL TEMPERATURE, BUT WHEN HE TOUCHED THEOTHER END WITH HIS BARE HAND HE RECEIVED A SECOND DEGREE BURN.

WHEN PICKING UP THE STEEL BAR HE EXPERIENCED A BURNING SENSATION TO HIS LOWER RIGHT ABDOMAN AREA POSSIBHERNIA. HE CONTINUED TO WORK ALL SCHEDULE SHIFTS THRU 8-25-03. STARTED LOOSING SCHEDULED WORK DAYS ON 8-26-03. BECAME A REPORTABLE INJURY ONT HE
WHILE UNLOADING FROM MANTRIP OUTSIDE AT END OF SHIFT THE RADIATOR CAP BLEW OFF ALLOWING HOT FLUID TO BLOW OUT.

OPERATOR HAS FULLY LOADED BUCKET IN RAISED POSITION, STARTS TO BACK AWAY FROM FACE, DOES NOT SEE MATERIAL FALL FROM FACE OVER FILLING BUCKET CAUSING LOAD TO PITCH FORWARD. OPERATOR SLIDES FORWARD IN SEAT, CHEST CONTACTED STEERING WHEEL, FOREHEAD CONTACTED WINDSHIELD.
INJURED WAS RETURNING TO THE FEEDER WITH A LOAD OF COAL AND HIT A BUMP IN ROADWAY CAUSING HIM TO INJURE HIS RIBS.

INJURED WAS SITTING IN THE OPERATOR'S SEAT OF A BATTERY POWERED MAC8, STEERING IT WHILE BEING PUSHED BY A SCOOP TO THE BATTERY CHANGING STATION. THE MAC8 BATTERIES WERE DEAD. WHILE IN ROUTE, T HE SCOOP PUSHED THE MAC8 OFF THE GROUND APPROX. 12"" JARRING THE INJURED CAUSING LOWER BACK PAIN.

ON 7/14/03 EE A SURFACE UTILITY WORKER RECEIVED 1ST & 2ND DEG BURNS TO 20% OF HIS BODY DURING ANINCIDENT INVOLVING HORSEPLAY. THE INJURY OCCURRED IN THE SURFACE SHOP AREA OF THE SOLAR SOURCES UNDERGROUND, MONROE CITY MINE. ON 7/14/03 AT 2:25 PM EE & ANOTHER EE WERE LOCATED IN THE SHOP TAKING A BREAK. WAS WORKING ON A SEALANT MACHINE PREPARING TO SEAL STOPPING. AS HE TURNED THE MACHINE ON THE VALVE WAS OPEN AND SEALANT STARTING STRIKING HIM IN FACE AND EYES. BEGAN MISSING TIME ON 7-16-03.

INJURED WAS CROSSING A TRACK RAIL AND TRIPPED AND FELL BECAUSE HE DID NOT LIFT HIS FOOT HIGH ENOUGH TO CLEAR THE RAIL.
EMPLOYEE WAS LIFTING ROLLS OF WIRE AND FELT A PAIN IN HIS LOWER BACK HE SOUGHT MEDICAL ATTENTION AND IT CAUSED LOST WORK DAYS.

INJURED WAS INSTALLING A TENSION CYLINDER IN NEW FORK WHEN THE CYLINDER CLIPPED OUT OF THE FORK STRIKING HIS LEFT KNEE.
EE WAS HAULING COAL FROM THE MINER TO THE FEEDERWITH A SHUTTLE CAR. WHEN HE GOT CLOSE TO THE CA BLE ANCHOR, THE SLACK ON THE CABLE CAME UP SUDDENLY AND A LOOP OF CABLE CAME IN THE DECK STRIKIN G HIM CAUSING THE INJURY. HE FINISHED THE SHIFT AND WENT TO THE DR. THE NEXT DAY AND HAS BEEN OFF WORK SINCE THEN.
EE WAS WORKING ON A PUMP GUARD & FELL WHEN HE STEPPED DOWN, CAUSING A HAIRLINE FRACTURE IN THAT AREA.
EMPLOYEE SHOULD HAVE ASKED FOR ASSISTANCE, INSTEAD HE WAS ATTEMPTING TO LOAD A 4 X 4 X 2 SCREEN, ELBOW POPPED, CONTINUED TO HURT AND WOULD NOT WORK PROPERLY.

EMPLOYEE WAS MOVING METAL WHEN BIT BY A SNAKE.
INJURED WAS OPERATING SHUTTLE CAR WHEN HE HIT HOLE IN ROADWAY CAUSING HEAD TO HIT CANOPY JAMMING NECK AND BACK.
ATTEMPTING TO REMOVE SADDLE FROM HEAD OF MINER, WHEN THE SADDLE BROKE LOOSE IT SWUNG DOWN & STRUCK HIS LEG. EMPLOYEE DID NOT START MISSING WORK UNTIL AUG. 21, 2003.
DRIVE OVER A SOFT SPOT, WHILE DRIVING A TRUCK CAME OUT OF HER SEAT BELT AND BODY HIT THE DOOR OF THE TRUCK.

EE WAS WORKING PUTTING A WIPER ON FEED BELT WHEN A ROCK WEIGHING APPROX. 50 LBS FELL OFF ABOVE HIM LANDING ON HIS FOOT FRACTURING HIS ANKLE. THE ROCK FELL APPROX. 4'.
UNPLUGGING BATTERY ON RAM CAR AND PLUG FIRED UP AND BURNT RIGHT HAND.
EE ALLEDGES TO HAVE HIT A BUMP IN THE CLAY SHED AREA WHILE OPERATING A 980 G CAT FRONT-END-LOADER CAUSING AGGRAVATION TO HIS NECK AND LOWER BACKTHIS IS AN AGGRAVATION FROM A PREVIOUS INJURY EE LOST TIME ON JULY 31.

EMPLOYEE WAS BREAKING BOLTS LOOSE ON A WA-600 WHEEL LOADER IN ORDER TO DISMANTLE HOSES AND JUNCTION BOX WHEN HE FELT A PULL IN HIS RIGHT SHOULDER.
WHILE TRAVELING TOWARD THE FEEDER IN A SHUTTLE CAR THE TRAILING CABLE CAME OUT FROM UNDER THE CAR STRIKING EMPLOYEE UNDER THE LEFT ARM. FAMILY DOCTOR SCHEDULED EMPLOYEE TO SEE A SPECIALIST ON 7/15/03 AT WHICH TIME IT WAS DETERMINED HE WOULD HAVE PHYSICAL THERAPY ON HIS ARM.
EMPLOYEE WAS HELPING FELLOW EE CHANGE A TIRE ON SCOOP. THEY BENT DOWN TO STAND THE TIRE UP, THE TIRE WAS LAYING FLAT ON GROUND, WHEN THEY STARTED TO LIFT, EE FELT PAIN IN HIS BACK.
EMPLOYEE WAS BEATING ON #1 RAW COAL SILO WITH A #10LB SLEDGE TRYING TO BREAK LOOSE STUCK COAL. HE NOTICED A BURNING ON HIS RIGHT SIDE, LOWER STOMACH. IT GOT WORSE AS THE NIGHT WENT BY. HE HAD TO HAVE HERNIA SURGERY & WENT OFF WORK 9-8-03.
EE WAS GETTING ON MANTRIP AND SLIPPED AND BUMPED HIS KNEE ON THE MANTRIP.
EE WAS CHECKING THE OIL ON THE 992C LOADER. HE OPENED THE ENGINE COMPARTMENT AND LOST HIS BALANCE & SLIPPED. AS HE TRIED TO CATCH HIMSELF HE FELL TO THE GROUND. RESULTED IN STRAIN TO LEFT SHOU LDER.
EE REPORTED AS HE GOT OFF HIS 3-WHEELER RIDE IT SOUNDED LIKE BIG MAX MANTRIP STARTING THE ENTRY CROSSCUT AS HE TURNED IT WAS A SHUTTLE CAR. IT WAS LOW IN THIS AREA S/C CAR OPERATOR DID NOT SEE EE BECAUSE OF HIGH AND THE OPERATOR WAS ON OTHER SIDE OF CAR. SHUTTLE CAR HIT HIS RIGHT LEG.
