A ROOF FALL WAS DISCOVERED BY THE SECOND SHIFT PRESHIFT EXAMINER ON THE NO.2 UNIT, 20TH EAST,#3ENTRY, SPAD 85+78.8. THE FALL OCCURRED IN THE PILLAR LINE AND THROUGH AN INTERSECTION. IT MEASUR ED APPROX., 100'X 17'X 8'. IT WAS IMPOSSIBLE TO DETERMINE IF THE FALL ORIGINATED IN THE PILLAR LINE OR IN AN INTERSECTION. IT FELL ON THE UNIT BELT TAILPIECE AND RATIO FEEDER.
SYNOPSIS: TWO CONTACT MECHANICS NOTICED TWO INDIVIDUALS ON ATVS NEAR WHERE A BULK TRUCK WAS PARKED. UPON INVESTIGATION THEY FOUND THE TRUCK ON FIRE. IT HAD TO BE EXTINGUISHED BY THE FIRE DEPT. IT APPEARED THE TRUCK WAS VANDALIZED & THE FIRE INTENTIONALLY SET. THE FRONT OF THE TRUCK INCLUDING THE CAB HAD COMPLETELY BURNED.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON THE 6 WEST LONGWALL IN A CROSSCUT BETWEEN #2 & #3 ENTRIES AT CROSSCUT #46. DIMENSIONS WERE ESTIMATED AT 70' LONG BY 6 TO 7' THICK BY 20' IN WIDTH.
ANCHOR BOLT BROKE ON GUIDE ROPES ON ESCAPE SHAFT. EVACUATED MINE AT 8:40 AM. BOLT WAS REPAIRED AND MINERS WENT BACK UNDERGROUND AT 10:40 AM.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT CROSSCUT #78 ON 2 EAST MAINS. FALL DIMENSIONS WERE ESTIMATED AT 20' WIDE BY 100' IN LENGHT BY 7 TO 8 FEET HIGH.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #5 ENTRY ON THE TILLEY SECTION. INTAKE 1 BREAK INBY SPAD #3140. THE FALL MEASURED 20'L X 20'W X 4 1/2'H. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERE D OFF. THE FALL WILL NOT BE REHABILITATED. VENTILATION OR PASSAGE WAS NOT AFFECTED.
PAGE PORTAL SERVICE CAGE- THIS FACILITY WAS DOWN FOR APPROX. 3 HOURS DUE TO A BAD POWER SUPPLY CARDIN THE POWER CONVERTER. THE CARD WAS CHANGED OUT AND HOIST WAS PUT BACK IN SERVICE. THERE WAS NO INJURIES RELATED TO THIS INCIDENT.
A FALL OF ROOF OCCURRED ON #5 UNIT, S.E. PANEL, #7 ENTRY, CROSSCUT #4 IN THE LAST OPEN CROSSCUT,35'INBY SPAD #39987. THE FALL WAS 65'LONG, 18'W.AND 6'THICK. NO PERSON, EQUIPMENT OR VENTILATION WERE AFFECTED.
THE VENTILATING FAN AT ""B"" SHAFT WENT DOWN DUE TO BEARING FAILURE. THE FAN, FAN SHAFT & BEARING WERE REPLACED. FAN WAS RETURNED TO OPERATION @ 8:30 PM 9/26/02.

A ROOF FALL WAS FOUND DURING THE WEEKLY EXAM ON 3 WEST MAINS IN THE NO. 2 ENTRY AT CROSSCUT #11. THIS AREA HAD BEEN BOLTED WITH 5' HIGH TECH BOLTS. FALL APPROX. 18' X 18' X 5' HIGH.
THE NO. 1 CONVEYOR BELT STOPPED AND THE NO. 2 BELT CONVEYOR DID NOT SHUT OFF THAT CAUSED A GOBB OUT AT THE HARD DRIVE AND MADE THE BELT START SLIPPE DIN THE DRIVE AND THAT IS WHAT CAUSED THE E E WERE EVUCATED FROMT HE MINE SAFETY.
FIRE FOAM SYSTEM BEING INSTALLED. MECHANICAL ISSUES CORRECTED.
AT #5 BELT INBY SPAD 939, A ROOF FALL OCCURRED AT THE ABOVE MENTIONED MINE SITE. THE SIZE OF THE FALL WAS 50' X 20' X 7'. THE STRATA IS MIXED AND THERE IS A COAL RIDER SEAM ABOUT 6' ABOVE MAIN SEAM. THIS AREA WILL BE REHABILITATED.
A ROOF FALL WAS FOUND AT 26+65 ENTRY 3 4LMS. FALL WAS IN FOUR WAY INTERSECTION AND MEASURED 20' X 20' X 10'. AREA WAS MINED 7-26-02 AND SUPPORTED WITH 6' RESIN PINS. NO INJURY OR PROPERTY DAM AGE RESULTED.
INNER DOOR FELL OFF OF TRACT. MEN OFF CAGE AT 1040 AM ELEVATOR DOOR REPAIRED AT 1115 AM.
THE ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES BECAUSE SOMEONE HAD PUT ANTISIEZE ON THE CAR DOOR LOCK SWITCH. THE ANTISIEZE WOULD NOT ALLOW THE SWITCH TO FUNCTION PROPERLY. THERE WAS NO ENTRAP MENT.

A MECHANIC WAS WELDING ON THE TUBING OF BORE MINER #2 IN THE LAST OPEN X-CUT IN PANEL L23, DURING A BELT MOVE. HE WAS USING A TMX 412 AS A CONSTANT MONITORING DEVICE. SPARKS FROM WELDING FELL TO THE FLOOR CAUSING A IGNITION OF A BLEEDER IN THE FLOOR. THE FOREMAN PUT IT OUT IN 2-3 MINUTES.

AT 5:15 AM THE HOIST WENT DOWN, WAS PULLING HIGH AMPS WHEN HOISTMAN ATTEMPTED TO MOVE THE SKIPS. AN IMMEDIATE INVESTIGATION REVEALED THAT THE INSIDE GUIDE SHOE, WAS ROLLED UP & WEDGED AGAINST GUIDES. THIS WAS CUT OUT, NEW GUIDE SHOE WAS INSTALLED, DURING THE SHAFT INSPECTION THEY FOUND A SPLICE BAR ABOUT 200' FROM THE BOTTOM OF SHAFT THAT NEEDED BOLTS. THE BOLTS WERE REPLACED.

THE WAS ASKED TO REMOVE SCREWS FROM A METAL ROOF DECK ON THE EAVE OF A ROOF. FOR REASONS UNKNOWN, THE MINER REMOVED HIMSELF FROM THE ELEVATED WORK PLATFORM (JLG AND GOT ON THE ROOF. HE ALSO VI OLATED COMPANYPOLICY IN PROCEEDING TO WORK IN AREAS OF THE ROOF WHILE NOT TIED-OFF. WHILE WORKING ON THE ROOF DECK, NOT IN FRONT OF THE JLG AND WHILE NOT TIED-OFF, HE FELL TO THE GROUND.

THE EE WAS HOLDING THE LIFTING BRACKETS UP TO ALLOW ANOTHER EE TO INSERT FORKLIFT WHEN HIS LEFT THUMB BECAME TRAPPED BETWEEN THE BRACKET AND THE FORK.
WORKER WAS LIFTING A PIECE OF FLAGSTONE OUT OF HIS WAY WITH ONE HAND WHEN THE STONE SLIPPED OUTOF HIS HAND AND PINCHED THE TIP OF LEFT MIDDLE FINGER OFF BETWEEN THE STONE AND HIS CUTTING WEDGE

EE WAS BEING TRAINED AS A STACKER. HE REACHED ON THE CHAIN BELT FOR A BAG OF MATERIAL; HIS GLOVE AND HAND WERE CAUGHT IN THE CHAIN BELT. RESULTED IN DISTAL AMPUTATION OF END OF RIGHT MID FINGE R. RETURNED TO WORK WITH RESTRICTIONS 9/17/02.

EMPLOYEE WAS INSPECTING HYDRAULIC HOSES ON THE DRILL HEAD AND LAID HIS HAND ON THE DRILL HEAD TO PULL HIMSELF UP WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS AND STRUCK HIM ON THE TIP OF THE RIGHT INDEX FINGER. THE NAIL WAS AVULSED FROM HIS FINGER. SURGERY WAS NEEDED TO REPAIR THE INJURY.

THIS EE WAS BEEN SUFFERING FROM LOW BACK PAIN FOR AT LEAST TWO YEARS DURING THIS PERIOD HE HAS RECEIVED THERAPY A COUPLE OF TIMES. DURING THE PAST WEEK 20-26 OCT HE TRIED TO WORK AND DENY HIS PAIN UNTIL IT BECAME UNEARABLE. HE IS NOW REPORTED TO THE STATE INSURANCE CORPORATION. WHICH WILL GIVE HIM ALL NECESSARY MEDICAL ATTENTION.
EMPLOYEE WAS WALKING #10 BELT, HIT HEAD ON GUARD AT TAIL PIECE. STARTED LOSING TIME ON 10/2/02.
EMPLOYEE HAD SET DOWN ON HI-LINE SUPPLY SLED TO EAT LUNCH. A PIECE OF DRAW ROCK FELL & HIT HIM.
COAL TRUCK DRIVER PULLED ONTO SCALES AT THE PREP PLANT. WHEN HE GOT OUT OF TRUCK HIS FOOT SLIPPED OFF TOP STEP OF FUEL TANK ABOUT 3'. HE FELL & HIS LEFT KNEE & SHIN STRUCK 6"" PIPE ON WALKWAY.
EE LOST POWER ON THE SHUTTLE CAR. WHEN HE WENT TO CHECK THE CABLE, THE CABLE ARCED, BURNING HIS LEFT HAND.
EE WAS PRYING A STEEL PLATE THAT WAS WEDGED IN THE CRUSHER, THE STEEL PLATE GAVE WAY AND LANDED ON EE'S FINGER.
EE WAS HELPING MOVE A DIESEL PUMP WHEN HE TOOK OFF THE DISCHARGE LINE, STEAM ESCAPED AND THE EE ALLEGEDLY GOT STEAM BURNS ON HIS CHEST, ARM, AND WRIST.
A ROCK FELL OFF THE TOP OF JAW AND HIT HIM ON KNEE WHERE HE WAS SHOVELING.
RIDING A THREE WHEEL BUGGY, HE RAN OVER SOMETHING IN THE ROADWAY AND CAUSED HIM TO TWIST HIS BACK.
WAS PUTTING 1ST BOLT IN 1ST TURN, ROCK FELL STRIKING HIS LEFT ELBOW.
DRILL STEEL STUCK IN THE ROOF. EE WAS REACHING FOR A WRENCH WHEN THE DRILL STEEL FELL OUT OF THE ROOF, STRIKING EE'S RIGHT INDEX FINGER.
WAS LOWERING TOWER ON THE LIGHT PLANT, HAND SLIPPED & THE WINCH HANDLE STRUCK HIS LEFT HAND.
EE WAS SHOVELING WASTE KAOLIN FROM AROUND #2 IMP MILL. HE FELT PAIN IN HIS LOWER ABDOMEN. FIRST DIAGNOSED AS KIDNEY STONES. CONTINUED VISITS TO DOCTOR NOW SHOWS A HERNIA 10-30-02.
OPENING HATCHES ON RAILCAR, HE STEPPED ACROSS OPENING & TRIPPED, CATCHING HIMSELF WITH HIS LEFT ARM CAUSING A STRAIN TO HIS ARM.
EE WAS STEPPING OVER THE FORKS OF A FORKLIFT, HEFAILED TO LIFT HIS LEG HIGH ENOUGH TO CLEAR THE FORK AND HIS BOOT CAUGHT THE FORK. HE FELL FORWARD HITTING HIS KNEE ON THE FLOOR. KNEE SURGERY ON 3/28/03.
EE SLIPPED WHILE DISMOUNTING GRADER. ABRASIONS TO LEFT LEG & RIGHT ARM. SLIGHT CRACK IN BONE IN RIGHT ELBOW.
EE WAS DUMPING PRODUCT FROM DUMP TRUCK WHEN THE CAB WENT UP IN THE AIR. EE HURT LOWER BACK WHEN CAB HIT THE GROUND.
THE EMPLOYEE INJURED HIMSELF WHEN HE DISMOUNTED A HAULAGE TRUCK. AS HE STEPPED TO THE GROUND HE TWISTED HIS BACK.

EMPLOYEE WAS REMOVING PARTS FROM THE INSTRUMENT REPAIR VAN. AS HE STEPPED OUT OF THE VAN HE FELT HIS KNEE POP. NO TREATMENT AT THE TIME. DIDN'T GET BETTER. HAD MRI FOUND TEAR OF LEFT KNEE MEDI AL MENISCUS. SURGERY ON 1/21/03. LOST TIME.
WRECKED DIESEL SUPPLY MOTOR INTO STEEL BEAM. FRACTURED RIB LEFT SIDE.
EE STATED THAT HE HAD BEEN PULLING HARD ON A RATCHET TO LOOSEN A BOLT AT ABOUT 0330. HE SAID THAT LATER HE BEGAN HAVING PAIN IN HIS ABDOMEN ON THE LEFT SIDE.
EE WAS SPLICING A CONVEYOR BELT, CUTTING THE BELT WITH A RAZOR KNIFE. HE WAS CUTTING TOWARD HIMSELF. THE KNIFE SLIPPED OUT OF THE BELT & WAS PULLED ACROSS RIGHT KNEE CAUSING A LACERATION ABOVE RIGHT KNEE CAP.
THE MINER WAS EXITING THE CAB OF THE CRANE WHEN HE SLIPPED AND HIT THE OUTRIGGER AND THEN THE GROUND. HE NEVER LOST CONSCIOUSNESS.
EE WAS WORKING UNDER CONTINUOUS MINER, AS HE CRAWLED OUT FROM UNDER THE MINER A PIECE OF ROCK FELL OUT FROM BETWEEN THE PERMANENT ROOF SUPPORTS AND STRUCK HIM ON THE RT. LEG, CAUSING A LG. CONTUSION TO THE UPPER PART OF HIS LEG.
STEPPED ON ROCK AND TWISTED FOOT. CARRYING 5 GALLON BUCKET STEPPED ON UNEVEN GROUND AND TWISTED FOOT.

WHILE EXITING A MANLIFT, EE WAS ""STOOPING"" DOWN TO GET UNDER THE MANLIFT BASKET SAFETY BAR. WHEN HE STOOD UP, HE FELT PAIN IN HIS LOWER BACK. THE DR. DIAGNOSED THE PAIN AS A LUMBAR STRAIN.
ROCK HIT EMPLOYEE ON HEAD WHILE BOLTING. STARTED LOSING TIME ON 10/11/02.
EE WAS LOADING ROOF BOLTS ON PINNER AND TWISTED HIS BACK.

EE WAS USING A COMEALONG TO RAISE A WINCH CABLE WHEN IT SLID BACK & MASHED HIS LEFT HAND.
EMPLOYEE WALKED THROUGH PULL THROUGH CURTAIN, AND RAN INTO BROW.
GRINDING A SCREW WITHING MILL, A PIECE OF METAL FLEW INTO HIS EYES.
MOVING STONE TO A STEEL PALLET, A STONE FELL ON HIM FROM BEHIND.
CLEANING SURFACE SHOP, MOVING METAL BAR, TWO MIDDLE FINGERS ON RIGHT HAND GOT SMASHED WITH THE METAL BAR. SMASHED AND BROKEN FINGERS.
EMPLOYEE WAS WALKING SLOPE WHEN HE STEPPED ON ROCK OR COAL, SLIPPED AND FELL, WHILE TRYING TO CATCH HIMSELF WITH HIS LEFT ARM HE SPRAINED HIS LEFT WRIST.
EMPLOYEE WAS MOVING A 4 INCH WATERLINE FROM A FLATCAR TO A SCOOP BUCKET. ONE END OF THE PIPE FELL OFF THE CAR AND WHEN IT HIT THE MINE FLOOR IT BOUNCED CAUSING HIS LEFT HAND TO BE MASHED AGAINST A SHARP PIECE ON THE SCOOP RAM.
EMPLOYEE STATED THAT WHILE SHOVELING ON BELT HE FELT A PAIN IN HIS LOWER BACK. BELT WAS ABOVE SHOULDERS.
EE WAS CARRYING REPAIR PARTS OUT OF THE PARTS HOUSE. HE BEGAN EXPERIENCING BACK PAIN DOING THIS.
EMPLOYEE STATED THAT HE WAS RIDING IN 66 BUS AS THE BUS WAS ATTEMPTING TO TRAVELING INTO THE ROCK DUST HOLE. THE OPERATOR OF THE BUS FAILED TO MAKE SURE THE SWITCH WAS THROWN FOR THE ""HOLE"" AN D THE BUS WENT TOWARD THE SOUTH END SWITCH AND HIT A MAIN LINE MOTOR.
EE STATED THAT HE WAS WALKING TO MAKE AN EXAMINATION OF MPL ""C"" WHEN HE SLIPPED ON A PIECE OF WET CONVEYOR BELT ON THE MINE FLOOR. HE SAID THIS CAUSED A STRAIN TO HIS LEFT KNEE BY TWISTING IT.
LOADING SUPPLIES ONTO ROOF BOLTER, AS HE PICK UP A BOX OF GLUE HE FELT SOMETHING GIVE IN HIS BACK.
MINER OPERATOR PICKED UP MINER CABLE TO MOVE AND BACK WENT OUT ON HIM. WORKED REMAINDER OF SHIFT AND WENT TO CLINIC THE NEXT DAY. DOCTOR TOOK HIM OFF WORK FOR OBSERVATION AND POSSIBLE SURGERY.
EMPLOYEE WAS POUNDING BOLT OUT OF CAP ON ROCK TRUCK, MISSED THE BOLT AND HIT HIMSELF ON LEFT FOOT.
EE WAS WELDING AN I-BEAM, HELD BY CLAMPS, TO A NEW HOPPER LID. HE ATTEMPTED TO TURN THE BEAM AND THE TACK WELD BROKE CAUSING THE BEAM TO FALL ON HIS UPPER & LOWER RIGHT ARM. THE INJURY REQUIRE D 4 DAYS OF LOST TIME TO ALLOW THE BRUISING AND PAIN TO SUBSIDE.

OPERATOR WAS TRYING TO PUT AIR CONDITIONER DIRECTIONAL COVER BACK IN PLACE AT THE SAME TIME AS HE WAS OPERATING THE LOADER. THE COVER SLIPPED & HIS LEFT HAND ENTERED THE OPENING. HIS LEFT RING FINGER CAME IN CONTACT WITH THE SQUIRREL CAGE FAN, LACERATING THE FINGERPRINT PORTION OF THE FINGER & ALSO RESULTING IN AN OPEN DISTAL TUFT FRACTURE ON THE END OF THE FINGER.
EE WAS ON A 4' PLATFORM LADDER HANGING A COME-A-LONG. THE HOOK OF THE COME-A -LONG HOOKED THE BOTTOM OF THE LADDER, CAUSING EE TO SHIFT HIS WEIGHT. WHEN THE LADDER FELL, IT CAUSED EE TO FALL.
EE WAS HANGING CURTAIN, BENT OVER AND TWISTED PULLING SOMETHING IN LOWER PART OF BACK. HE CONTINUED TO WORK UNTIL 9/25/02 WHEN HE CONSULTED A DOCTOR.
EE ALLEGES HE STRAINED HIS BACK WHILE SHOVELING BELT. EE CONTINUED TO WORK UNTIL 5-29-03 AND THEN STARTED MISSING TIME DUE TO INJURY.
EE WAS LOADING 8' ROOF BOLTS ONT HE ROOF BOLTER WHEN HE FELT A PAIN IN HIS MIDDLE BACK.
WHILE DOING ROUTINE MAINTENANCE ON A BACK DUMP, EE'S FOOT SLIPPED OFF THE LADDER STEP, GOING BEHIND IT CAUSING HIM TO FALL BACKWARDS, LEAVING HIM DANGLING UPSIDE DOWN BY HIS LEFT LEG, TWISTING HIS KNEE. CONDITIONS WERE WET AFTER HARD RAIN.
EMPLOYEE WAS LOADING BELT STRUCTURE ONTO SCOOP AND BELT RAIL BOUNCED UP PINCHING LT. LITTLE FINGER TOP BETWEEN BELT RAIL AND STAND.

EE WAS SITTING ON THE RIB EATING HIS LUNCH, A PIECE OF COAL BLEW OUT HITTING HIS RT. HAND.
EE WAS CHECKING AN ORFICE FITTING ON A HYD.REEL ON A MRS MOBILE ROOF SUPPORT. HE STARTED THE MACHINE & THE PRESSURE FROM THE PUMP CUT HIS RIGHT INDEX FINGER.
A PIECE OF ROCK FELL OUT OF THE MINE ROOF IN BETWEEN THE ROOF BOLT & RIB. THE PIECE OF ROCK STRUCK EE'S HEAD.

EMPLOYEE STATED HE WAS CHANGING A BURN LOOP FILTER BRACKET WHICH WEIGHED BETWEEN 30-50 LBS. WHILE LIFTING THE BRACKET HE FELT A STRAIN AND A PULL IN HIS RIGHT GROIN.
BUMPED RIGHT KNEE CAP ON STEERING COLUMN OF LOADER.
INJURED WAS BACKING MINER UP, WHEN A PIECE OF RIB ROLL HIT EMPLOYEE ON RIGHT SIDE OF THE HEAD, AND UPPER BACK WHICH RESULTED IN KNOCKING HIM TOTHE MINE FLOOR.
EE WAS SETTING IN A JB LONG MANTRIP, RIDING TO THE SECTION. HE CROSSED HIS RT. LEG OVER HIS LT.LEG AND FELT HIS KNEE ""POP"". EE CONTINUED TO WORK UNTIL 11/21/02, WHICH WAS HIS FIRST DAY AWAY FROM WORK DUE TO THE INJURY.
EMPLOYEE STATED HE WAS WALKING DOWN THE #3 RETURN SLOPE ENTRY SLIPPED AND TWISTED LEFT KNEE CAUSING A POSSIBLE STRAIN TO HIS KNEE.
WAS MOVING SCREENS FROM A PICK UP INTO OR ON A TRAILER, WHEN SOMETHING FLEW INTO THE EYE. IT WAS A WINDY DAY.
EE WAS CHECKING THE WASH PLANT BEFORE START-UP, CLEANING THE FEED CONVEYOR AND ATTEMPTING TO REMOVE A PIECE OF WOOD FROM THE BELT. THE EE PINCHED HIS FINGER BETWEEN THE SIDE OF THE HEADSTOCK & THE FEED BOX.
WHILE WALKING UP THE BELT ENTRY TO THE SECTION, A PIECE OF DRAW ROCK FELL OUT FROM BETWEEN THE RIB AND THE SECOND ROW OF BOLTS STRIKING EE ON THE HEAD. HE WAS TAKEN TO THE HOSPITAL WHERE HE WA S TREATED AND RELEASED. HE WORKED UNTIL 9-25-02 WHEN HE DEVELOPED A HEADACHE AND NAUSEA. HE WAS THEN DIAGNOSED WITH A CONCUSSION.
HANGING MINER CABLE, HANGER BROKE AND FELL STRIKING HIM ON THE BACK OF NECK. STARTED MISSING WORK ON 9/23/02.
EE ALLGES HIS RIGHT KNEE WAS STRUCK BY THE HEAD OF THE ROOF BOLT MACHINE BEING OPERATED.
EMPLOYEE ALLEGES HE STRAINED HIS BACK LOADING SUPPLIES FOR ERECTING A SEAL. (NOTE: EMPLOYEE WORKED UNTIL 01-17-03 THEN STARTED MISSING TIME FROM WORK.)

EE WAS POSITIONED ON CONVEYOR TRUSS BOLTED TO BENT LEG #2. THE BOLTS WERE REMOVED CAUSING TRUSS TO FALL INSIDE THE BENT LEG WHICH TIPPED 45 DEGREE. HE LANDED ON THE GROUND AND DURING THE FALL PINCHED L. ARM WHICH SWELLED.
DRIVING MAC 8. STEERING WHEEL SLIPPED OUT OF HAND HITTING BACK OF HAND W/KNOB.
EE WAS GETTING GLUE OFF BUCK OF ROOF BOLTER TO TAKE TO THE FRONT WHEN HE RAISED UP AND FELT PAIN IN LOWER BACK.
WHILE RIDING THE MANTRIP IT HIT A BUMP AND THRU THE VICTIM INTO THE BUGGY FRAME.
EMPLOYEE WAS WELDING A GATE FOR A SPLITTER BOX. THE LADDER WAS NOT SET UP PROPERLY AND HE FELL.
PICKED UP A CHEEK PLATE AND IT SLIPPED OUT OF HIS HAND STRIKING HIS FOOT AT THE INSTEP.
HE KNELT DOWN AND HIS KNEE HIT A PIECE OF ROCK SURGERY CAUSING PAIN. HE CONTINUED TO WORK AND HAD CATILAGE SURGERY ON 12-20-2002. THE MINE WAS DOWN FOR VACATION FROM 12-2-2002 UNTIL 1-2-2003.
UNDER CAT 973 LOADER SERVICING EQUIPMENT. WHEN EMPLOYEE CAME OUT FROM UNDER EQUIPMENT AND STOOD, HE TWISTED LOWER BACK.
THE EE WAS FIREBOSSING THE TRACK HAULAGE WHEN THE TROLLEY POLE ON HIS RIDE CAME OFF THE TROLLEY WIRE. HE WENT TO PUSH DOWN ONT HE TROLLEY POLE TO PUT IT BACK ON THE WIRE, WHEN HE FELT A PAIN I N HIS LEFT SHOULDER.
EE WAS HELPING A CO-WORKER LIFT A SECTION OF STEEL PLATE FROM GROUND LEVEL TO A WORK BENCH. AT ABOUT 9:00 P.M. OF 9/19/02 HE EXPERIENCED PAIN & SWELLING OF THE RIGHT TESTICLE. THIS INJURY WAS INVESTIGATED & DETERMINED TO BE WORK-RELATED ON 10/31/02.
EE WAS WALKING DOWN A SLOPING SURFACE AND LOST HIS FOOTING AND TWISTED HIS BACK.
WHEN GETTING OUT OF THE CRUSHER HE WAS WORKING ON HE SLIPPED HITTING SHOULDER ON FRAMEWORK. HAS BEEN WORKING WITH SORE WENT TO DOCTOR BUT NOT BETTER. NO 1-14-03 FOUND TORN ROTOR CUFF
SURFACE, SLURRY POND - EE WAS WALKING WHEN HE FELL IN A HOLE STRAINING LEFT KNEE. GOB WAS WET, MAKING WALKING SURFACE MUDDY. ALSO, THERE WERE NO LIGHTS IN THE AREA.
EE SLIPPED WHILE EXITING A LOADER, HIT HIS TAIL BONE ON LOADER STEP AND FELL TO GROUND.
INJURED STATED THAT WHILE HELPING CHANGE A GOLF CART TIRE HE FELT A PULL IN HIS LOWER BACK. INJURED WENT TO THE DR. AND RECEIVED MEDICATION.
EE DIDN'T KNOW OF ANY ONE INSTANCE THAT MAY HAVE RESULTED IN THIS INJURY, HE CLAIMS THAT HIS LOWER BACK JUST GOT SORE & FELT STRAINED ON THIS PARTICULAR DAY. EE WAS GETTING OUT OF THE POWDER TRUCK WHEN HIS FOOT SLIPPED, WHEN HE CAUGHT HIS WEIGHT WITH HIS HAND HOLDING THE HANDRAIL IT PULLED HIS SHOULDER.
EMPLOYEE CUT LITTLE FINGER ON LEFT HAND, REQUIRED 5 STITCHES, STARTED LOSING TIME 9/19/02.
WHILE HOOKING UP TRACTOR TO TRAILER, HE TWISTED LEFT KNEE.
CARRYING A HYDRAULIC PUMP FOR CONE CRUSHER. STEPPED ON ROCK AND PULLED TENDON IN LEFT ANKLE AND LEFT SHOULDER.
WHILE WORKING ON FLY ASH BIN, HOSE WAS PRESSURRIZED AND FLY ASH MATERIAL GOT INTO EYES.
EE WAS PULLING A MINER CABLE. HE STEPPED ON SOME UNEVEN GROUND & TWISTED HIS ANKLE.
EE WAS OPERATING SCRAPER, STRIPPING BASALT IN WEST PIT, OTHER EE WAS OPERATING PUSH CAT. AS EE IN SCRAPER WAS DROPPING BOWL, OTHER EE BUMPED INTO HIM HARD FROM BEHIND, CAUSING SCRAPER TO GO OVER A LARGE PIECE OF BASALT, JARRING EE A SECOND TIME, STRAINING HIS NECK AND BRUISING HIS RIBS AND HIPS.
THE INJURED EMPLOYEE WAS ATTEMPTING TO FUEL THE MAIN WATER PUMP. HE STEPPED UP ON THE LEFT REAR TIRE OF THE TRAILER THAT THE PUMP IS MOUNTED ON, TO REACH THE FUEL TANK. THE TIRE WAS WET FROM D EW, HE SLIPPED AND FELL OFF OF THE TIRE, LANDING ON HIS BACK ONTO A MOUND OF SAND OVER A PIPE IN THE GROUND.
EE STATED HE WAS LIFTING A 5 GALLON BOX OF WATER TO PLACE IT ON A STAND AT CHEST HEIGHT. SOON AFTER THIS LEFTING EE STATED SOME DISCOMFORT IN HIS UPPER LEFT BACK. HE DID NOT FEEL IT WAS BAD EN OUGH TO GO TO A DOCTOR AT THAT TIME. THE PAIN, HE STATED, CONTINUED AND MOVED DOWN HIS LEFT ARM.
THE EE WAS DOING HIS REGULAR MAINTENANCE WORK & APPARENTLY HE WENT UP THE HIGHEST PART OF THE PLANT WHICH IS APPROX. 8' HIGH. HE SLIPPED OR TRIPPED & FELL RECEIVING INJURIES TO HIS SIDE & LEG.
EMPLOYEE WAS HAND ROCK DUSTING #5 BELT 4 CROSS CUTS FROM THE BELTHEAD. THE HEIGHT WAS APPROX 4-4 1/2 FT WHILE CARRYING 40 LB BAG OF ROCK DUST HE FELT A BURNING PAIN IN HIS GROIN AREA, RESULTIN G IN A MUSCLE STRAIN.
POUNDING ON COAL FEEDER WITH SLEDGE HAMMER. STRAINED LOWER BACK.
EMPLOYEE WAS CLEANING THE ALKALI BY PASS AND RESTRICTORS AREA AT THE PREHEATER TOWER STRIKING AN IRON BAR WITH A SLEDGE HAMMER. AFTER A FEW HOURS HE BEGAN TO FELL PAIN IN HIS BACK AND RIGHT SH OULDER.
CHANGING CAGE ON WHEEL CRUSHER. WHEN TO PULL LAST BOLT FROM HUB ON OLD WHEEL AND DID NOT HAVE CRANE ATTACHED TO TOP OF WHEEL TO IMMOBILIZE IT. WHEEL TURNED WHEN RELEASED AND PINCHED EE'S LOWE R LT. HAND BETWEEN WHEEL-CAGE AND HUB AND IMPACTSOCKET. CRUSHED LITTLE FINGER AND LOWER HAND.
WHILE UNPLUGGING CRUSHER, REEL ROLLED AND PINCHED LOWER LEG.
HE WAS REBUILDING ROLLER FRAMES FOR D11 TRACTOR. TAKING SNAP RINGS OUT OF BOGGIE PIN COVERS WHEN HE GOT A SMALL PIECE OF METAL IN HIS LEFT EYE.
WHILE SERVICING CONE CRUSHER, EMPLOYEE SLIPPED OFF SCAFFOLDING AND FELL TO GROUND, BREAKING HIS LEG.
AS EMPLOYEE WAS OPENING THE ELEVATOR DOORS, THEY STUCK. HE ATTEMPTED TO RELEASE THE DOORS BY PULLING DOWN & THEN PUSHING THEM UP. WHILE PUSHING THE DOORS UP HE CONTINUED TO HOLD ONTO THE STRAP WHICH ALLOWED HIS FINGER TO BE CAUGHT BETWEEN THE DOOR & FRAME.
WHEN THE EMPLOYEE OPPENED THE DOOR TO THE BACKHOE, THE DOOR CAME FLYING OPEN AND JAMMED HIS LEFT WRIST.
A PIECE OF ROCK FELL ACROSS BOOM ON BOLTER & BROKE IN HALF. AS EMPLOYEE LEANED BACK TO GET OUT OF THE WAY, HE SLID OFF THE SEAT OF THE BOLTER.
EMPLOYEE WAS RUNNING CAT 988F LOADER IN THE RAINWHEN SHE SLIPPED WHILE CLEANING THE WINDSHIELD. SHE HAD MINOR INJURIES (BUMPS AND SCRAPES) TO HER LEFT LEG.
INJURED SAID HE WAS HANGING THE CONTINUOUS MINERS CABLE FROM THE MINE ROOF AND AS HE LIFTED THE CABLE, HE FELT A SHARP PAIN IN HIS BACK.
INJURED HAD OPENED A DOOR THAT RAISED FROM THE BOTTOM AND PROPPED IT UP WITH A ROOF BOLT. AS SOON AS HE PASSED UNDER THE DOOR, HE KNOCKED THE PROP LOOSE, CAUSING THE DOOR TO FALL AND STRIKE HI S BACK. THE DOOR WEIGHED APPROX. 70# & FELL APP. 2 FEET.
CONTRACTOR WAS REMOVING PLYWOOD WALLS FROM EACH SIDE OF CEMENT RETAINER WALL AROUND RARE EARTH RECOVERY SYSTEM. WHILE THIS WORK WAS IN PROGRESSANOTHER CONTRACTOR WAS REMOVING TEMP PIPE BRACES FROM INSIDE RETAINER WALLS AND REMOVING THEM TO OUTSIDE, THE METAL BRACE FELL OFF ON ONE END AND STRUCK MAN ON RIGHT SHOULDER AND BACK AREA.
EMPLOYEE WAS ROLLING A ROCK OFF A BLUFF. WHEN HE STRAIGHTENED UP HE HAD A MUSCLE SPASM IN HIS BACK RESULTING IN A LUMBAR STRAIN.
ROCK DISLODGED FROM ROOF & STRUCK MIDDLE FINGER ON LEFT HAND.
EE WAS HELPING REPLACE BELT ROLLER ON THE STACKER WHEN HE FELT PAIN IN THE LOWER PART OF HIS BACK. HE LEFT WORK & WENT TO THE DOCTOR. THIS IS A LOST TIME ACCIDENT AT THIS TIME THE DOCTOR THINK S HE HAS A PULLED MUSCLE.
FOUND LAYING BESIDE ROCK TRUCK UNCONSCIOUS.
WHILE LIFTING AN HYDRAULIC RAM FROM A TRUCK BOX ONTO A DRAGLINE SHOE, THE EMPLOYEE'S MIDDLE LEFT HAND FINGER GOT PINCHED. THE INJURY TO HIS FINGER REQUIRED MEDICAL TREATMENT.
EMPLOYEE STATED HE WAS WORKING ON CONVEYER #16 REPLACING ROLLERS, WHEN HE STEPPED FROM CONVEYER TO WALKWAY, HE TWISTED HIS LEFT KNEE AND FELT PAIN, BUT KEPT WORKING. PAIN CONTINUED, AND HE RE PORTED IT THE NEXT DAY.
EE WAS DOING CLEAN-UP AND WHILE MOVING A FLOOR STAND, HE FELT PAIN IN LOWER STOMACH. EE WENT TO DR. ON 9-18-02, WHEN HE WAS DIAGNOSED WITH A NAVEL HERNIA.
THE EMPLOYEE WAS ASSISTING A FELLOW WORKER HANG THE CONTINUOUS MINER CABLE. HE LATER BEGAN TO EXPERIENCE BACK PAIN.
DUE TO FOG, DRIVER LOST SITE OF ROAD AND TURNED THE STEERING WHEEL TO SHARPLY AND OVERTURNED THE TRUCK.
EE WAS HELPING TO MOVE A MINER, AS PART OF THIS MOVE HE WAS HELPING TO HANG THE MINER CABLE. HE FELT SOME BACK PAIN AFTER THIS EVENT AND IT CONTINUED TO GET WORSE AS HE BOLTED THE REST OF THE DAY. HE SWITCHED TO A COAL HAULER TO HELP EASE THE STRAIN ON HIS BACK. EE CONTINUED TO WORK & ON 10/26/02 HIS PROBLEM FLARED UP AGAIN HE WAS TAKEN TO THE HOSPITAL AND BECAME AN LTA ON 10/28
EMPLOYEE WORKING WITH GILLATEEN HAD TOO MANY SLABS OF ROCK ON ROLLER TABLE, ONE ROCK PUSHED A CENTER ROCK OFF ON EMPLOYEE'S FOOT, BREAKING ALL FIVE BONES IN FOOT.
EMPLOYEE SLIPPED GETTING ON THE BACKHOE. AS HE WAS FALLING, HE GRABBED THE HANDRAIL AND PULLED A MUSCLE IN HIS GROIN. EE DID NOT MISS WORK UNTIL 9-24-02.
EE WAS CLEANING THE WINDSHIELD ON A CAT 980G FRONT END LOADER, HE SLIPPED AND FELL APPROX. 10' TO THE GROUND FRACTURING HIS LEFT WRIST.
EMPLOYEE WAS LIFTING A CONVEYOR BELT ADJUSTER & FELT PAIN IN BACK.
EE WAS ASSEMBLING NEW STEEL INTO A GIARD UTLIZING THE WELDING TABLE AS A WORKBENCH. THE EE WAS WORKING ON THE CONSTRUCTION UNDER THE WELDING TABLE OVERHANG. WHEN EE RAISED UP TO A STANDING POSITION, UNDER HIS OWN POWER, HIS LEFT SHOULDERSTRUCK THE WELDKING BENCH.
WHILE CUTTING A PIECE OF HOSE, HAND SLIPPED CAUSING LACERATION TO LEFT KNEE.
ROCK WAS PUSHED OFF HIGHWALL & HIT THE EDGE OF HIS MACHINE & JARRED HIM, INJURING HIS SHOULDER. (WC PENDING INVESTIGATION, EE DIDN'T REPORT TILL MUCH LATER.)
EE WAS SHOVELING DIRT AND FELT PAIN IN HIS BACK AND HIP RIGHT SIDE. HE WENT TO THE DR COMPLAINING OF PAIN IN THE LOWER BACK. HE STATED THE ONSET TO BE 9-16-02. HE CONTINUED TO WORK SINCE THE F IRST INCIDENT ON 1-10-03.
EMPLOYEE MOVED A ROCK OFF BELT STRUCTURE AND FELT A BURNING PAIN IN GROIN AREA. RUPTURED HERNIA - WILL REQUIRE SURGERY. STARTED MISSING 9/23/02.
EE WAS GETTING DOWN OFF LOADER TO GET FUEL. WHEN HE STEPPED OFF THE LADDER, ONE FOOT WAS IN THE MUD. HE SLIPPED & FELL TWISTING HIS LEFT KNEE.
EE STATED THAT WHILE CLEANING THE NO.2 TAILPIECE, HIS KNEE GAVE IN CAUSING HIM TO FALL AGAINST THE TAILPIECE, INJURING HIS LOWER BACK & NECK.
INJURED WAS OPERATING CAT STICK STEER LOADER (BOBCAT). HE PUSHED HOT LIME AND WATER INTO A PILE WHEN BOTH HOT LIME AND STEAM CAME IN ON HIM. HE JUMPED FROM THE CAB OF THE MACHINE AND LANDED IN HOT LIME.
COMING DOWN THE ACCESS TO THE SCALPING SCREEN HE HAD BEEN CHANGING OUT THE MECHANISM ON WHEN HIS FOOT SLIPPED. HE LOST HIS BALANCE AND FELL BACKABOUT 2 1/2 FEET TO THE GRATED FLOOR. HE STUCK O UT HIS LEFT ARM TO CATCH HIMSELF.
EE CLAIMS WRIST AND KNEE INJURY OCCURRED BECAUSE SHE STRUCK A WOODEN BENCH. SHE CLAIMS SHE STRUCK THE BENCH BECAUSE SHE HAD AN ANXIETY ATTACK DUE TO WEARING A RESPIRATOR. SHE WAS WEARING THE RESPIRATOR BECAUSE OF A PREVIOUS SITUATION WHERE HER DR. SAID SHE COULD NOT BE EXPOSED TO CHEMICALS, DUST OR PAINTS.
WHILE PREPARING TO HELP DO BELT MAINTENANCE WORKTHE EMPLOYEE WAS CARRYING AN ACETYLENE CYLINDER TO THE SITE. THE ROOF HEIGHT WAS LOW, EMPLOYEE COMPLAINED OF LOW BACK PAIN.
THE INJURED WAS IN THE WALKWAY UNDER THE SHIELDS WHEN A ROCK CAME OUT OF THE FACE, SLID DOWN THE TOP OF THE SHEARER AND STRUCK HIM IN THE RIGHT LEG. HE RECEIVED A LACERATION WHICH REQUIRED NUM EROUS SUTURES INSIDE THE WOUND AND 8 STAPES TO CLOSE THE WOUND.
CONTINUOUS MOVEMENT OF LOADER SIDE TO SIDE AND BOUNCING CAUSED BACK STRAIN.
EE WAS EXITING DOZER, SLIPPED ON TRACKS AND STRUCK THE DOZER TRACK WITH HIS UPPER TORSO AND BACK.
REINJURY OR PAIN IN LOWER BACK REGION FROM PREVIOUS INJURY 12/27/01.
EE SLIPPED WHILE CLIMBING DOWN LOADER JERKING HIS LEFT ARM. THIS CAUSED A JERKING MOTION THAT POSSIBLY CAUSED TEAR IN HIS ROTATAR CUFF. EE RETURNED TO FULL DUTY ON 10/15/02. LATER HAD MORE PAIN IN SHOULDER, & CURRENTLY ATTENDING PT BUT STILL HAS LIMITATION ON ARM MOVEMENT. EXPECTED TO REACH END OF TREATMENT IN 2 MONTHS.
WHILE REACHING DOWN TO MOVE PUMP HE FELT SOMETHING IN HIS BACK PULL, HE STATES THAT HE FLUNG HIMSELF UPRIGHT IN REACTION AND HIT HIS HEAD ON THE MINE ROOF.
SHE WAS WALKING DOWN THE STEPS FROM THE CHANGE-OUT TRAILER AND MISSED A STEP.
EMPLOYEE SUSTAINED A SEVERE HEAD INJURY WHEN HE WAS STRUCK ON THE LEFT SIDE OF HIS HARD HAT BY A PIECE OF METAL. THE INJURY OCCURRED WHEN A PUMP EXPLODED AFTER A SCREENBOX HAD BEEN CLEARED AND THE PUMP RESTARTED.
EE WAS OPERATING A LOADED HAUL TRUCK WHEN HE HIT A POT HOLE IN THE ROAD.
EMPLOYEE WAS SETTING TIMBERS, THE RIB FELL, PUSHING HIM INTO A KENN STOPPING, CUTTING HIS RIGHT EAR.
EMPLOYEE WAS WELDING ON THE FINE COAL DRAG CHAIN AND RECEIVED AN ELECTRICAL SHOCK WHILE WELDING ON PINS OF 678 CHAIN. THE CAUSE OF THE ELECTRICAL SHOCK IS UNKNOWN. THERE WAS NOTHING EVIDENT WH ICH WOULD HAVE BEEN THE CAUSE.
EE STATED HE WALKED AWAY FROM TOOL CAR, STEPPED IN HOLE, TWISTING ANKLES.
EE WAS GETTING READY TO SERVICE A MINER WHEN HE TURNED AROUND AND WALKED INTO A LOW HANGING PIN BOARD JAMMING HIS NECK IN THE PROCESS. HE STATED AT A LATER DATE THAT THE USED HIS RIGHT ARM TO CATCH HIMSELF AS HE FELL BACKWARD AFTER COMING IN CONTACT WITH THE ROOF BOARD HE CONTINUED TREATMENT OFF AND ON OVER THE NEXT SEVERAL MONTHS AS THE DR. TRIED TO DETERMINE THE CAUSE OF HIS DISC
HELPING ON MINER CABLE, FEET GOT CAUGHT IN MINER CABLE AND HE TRIPPED AND FELL.
THE EE WAS INSTALLING CABLE AT AND AROUND A NEW BELT HEAD. HE WAS USING A STEP LADDER CLIMBING AND LIFTING CABLE. HE REPORTED A BACK PAIN ON MONDAY 9-16-02. ON 9-17-02 HE BEGAN MISSING TIME.
EE WAS CARRYING A TOP ROLLER ASSEMBLY CRADLE CLING THE A WEST BELT WITH ANOTHER EE TOP WAS LOW INT HE AREA AND HE WAS WALKING BENT OVER. HE TWISTED HIS KNEE. THERE WAS NO DAMAGE OR IMPAIRMENT TO THE OPERATION OR TO ANY EQUIPMENT. NO OTHER FAULTER WERE INVOLVED.
EE WAS PICKING UP BELT IDLERS & THROWING INTO BACK OF PU WHEN HE FELT A POP IN HIS RIGHT SHOULDER & FELT PAIN. EE DID NOT START MISSING WORK UNTIL 3/5/03 WHERE SURGERY WAS PERFORMED TO REPA IR SHOULDER
EE WAS CHANGING A DRILL BIT ON THE BOLTER WHEN THE PIPE WRENCH SWUNG AROUND, STRIKING THE EE INT HE NOSE, WHICH REQUIRED 3 SUTURES.
HELPING ANOTHER EMPLOYEE LIGHT A TORCH, HE THEN BURNED HIS LEFT HAND.

EMPLOYEE WAS INSTALLING 48"" ROOF BOLTS IN AN AREA WHERE THE MINING HEIGHT WAS 37"" WHEN HE STRAINED HIS BACK BENDING A BOLT.
AS THE LOADER LOADED MATERIAL INTO THE TRUCK, THE LOADER BUCKET DROPPED STRIKING THE TRUCK.
CATCHING 50LB BAGS OF TALC HE FELT PAIN IN LEFT WRIST & FOREARM. ON MONDAY & TUESDAY, SEPT. 16TH& 17TH, HE WAS MUCKING IN THE MILL & FELT PAIN TO LEFT WRIST. LOST TIME STARTED 9/18/02.
EE WAS PARKING DOZER AT THE END OF HIS SHIFT AS HE WAS STEPPING OFF THE DOZER, HE SLIPPED ON THE DOZER TRACK AND FELL TO THE GROUND BREAKING HIS RIGHT HIP.
EE WAS FUELING A CAT LOADER, HE HAD TAKEN THE FUEL HOSE OFF THE LOADER... THE FUEL VALVE ON THE LOADER STUCK OPEN, DUMPING FUEL INTO EE'S FACE &UPPER BODY.
EE AND COWORKER WERE IN THE PROCESS OF TAKING A OUTBOUND TRAIN TO ALCHEM JUNCTION. EE WAS ON THE FRONT OF THE ENGINE WHEN THE TRAINE DERAILED AND THE ENGINE WENT TO THE GROUND. WE BELIEVE EE J UMPED CAUSING A FX TO THE LEFT LEG AND ARM.
EE EXPERIENCED NECK & LOWER BACK PAIN WHILE SHOVELING HEAVY ROCK.
ON 9/13/02 AROUND 2 PM EE WAS COMPLETING WORK OF PRESSURE WASHING GREASE. HE THEN BEGAN TO CLEAN HIS RUBBER BOOTS WITH THE PRESSURE WASHER. THE PRESSURE WASHER PUT A HOLE ON TOP OF BOOTS AND B URNED FOOT.
WORKER CLAIMED THAT RIDING AROUND IN A 631 SCRAPER WITH A LOOSE SEAT CAUSED INJURY TO HIS BACK.
MINER WAS DRIVING SHUTTLE CAR OVER ROUGH SPOTS HIT BUMP AND JAMMED NECK ON CANOPY OF BUGGY. RETURNED TO WORK THINKING HE WOULD FEEL BETTER. HEADACHE GREW WORSE AND NUMBNESS IN HANDS INCREA SED. MINER DROVE SELF TO HOSPITAL. HAD SURGERY ON C5 AND C6 DISKS.
EE WAS OPERATING A FAIRCHILD 35C SCOOP, CLEANINGROADWAYS ON THE CONSTRUCTION AREA OF THE NEW #1 SECTION. HE WAS TRAMMING THE SCOOP IN REVERSE WHEN THE OPERATOR SIDE REAR TIRE RAN OVER AN OLD TIMBER LYING IN THE ROADWAY. THE END OF THE TIMBER CAME UP INTO THE DECK AND STRUCK EE IN THE RIBS.
EE JUMPED OFF THE BED OF A WATER TRUCK.
WHILE WORKING WITH ANOTHER EE TO MOVE A PORTABLE WATER PUMP, THE PUMP BEGAN TO FALL. EE ATTEMPTED TO KEEP THE PUMP FROM FALLING. IN DOING SO HE INJURED HIS RIGHT SHOULDER.
ROCK FELL FROM THE TOP STRIKING PERSON IN THE HEAD AND NECK.
EMPLOYEE LIFTING BULL GEAR OUT OF OIL BATH AND FELT PAIN. EMPLOYEE DID NOT STOP WORK UNTIL 10-1-02.

EE GOT ON TOP OF THE HOT MIX PLANT & ATTEMPTED TO WELD SOMETHING & TOOK LID OFF & HE THEN REACHED DOWN TO CLOSE LID AGAIN SHIRT CAUGHT FIRE. HE JUMPED DOWN INJURING FOOT & DISLOCATING TOES. WI TNESS GRABBED HIS SHIRT & RIPPED IT OFF.
EE EXPERIENCED PAIN IN HIS BACK AND SIDE WHILE TRYING TO GET OUT OF THE SCOOP.
EMPLOYEE WAS CLIMBING TO GET IN HIS ROCK TRUCK WHEN HE FELT A SHARP PAIN IN HIS LOWER LEFT BACK, ARM AND LEG.
EE SLIPPED ON STEP WHILE CLIMBING INTO TRUCK CAB AND FELL AGAINST THE SIDE OF THE TRUCK.
EMPLOYEE WAS CLOSING THE DOOR ON THE PREP PLANT ELEVATOR. HE SMASHED HIS HAND BETWEEN THE DOORS AS THEY CAME TOGETHER. EMPLOYEE FAILED TO KEEP HANDS CLEAR OF THE PINCH POINT. NO OTHER FACTORS WERE INVOLVED.

EMPLOYEE WAS FUEL LOADER @ FUEL STATION WHEN HE SLIPPED & GRABBED HAND RAIL W / LEFT HAND PULLING LT. SHOULDER.
EE STATED THAT HE BENT OVER TO MOVE MINER CABLE OUT OF HALLWAY, FEELING PAIN IN LOWER BACK. WENT TO DOCTOR ON 9-12-02 STARTED MISSING WORK ON 9-18-02.
THE BED OF AN ARTICULATE HAUL TRUCK ROLLED OVER ON ITS RIGHT SIDE.
WORKER STANDING ON LADDER, REACHING OVER HEAD TO TAKE BOLTS OUT OF EQUIPMENT. BOLT WOULDN'T COME OUT - HIS GRIP SLIPPED CAUSING HIS BODY TO TWIST TO THE SIDE. THE MUSCLES IN HIS BACK KNOTTED U P.
EE WAS USING A PRY BAR TO REMOVE A PIECE OF ROCK FROM THE FEEDER. THE BAR WAS CAUGHT BETWEEN THE ROCK AND A FEEDER FLIGHT. CAUSING THE BAR TO STRIKE THE EE.
A ROCK SPLITTER MOVED A LARGE ROCK BY HIMSELF, RESULTING IN A STRAINED MUSCLE IN HIS ABDOMEN.
WAS HYDROSEEDING, C HOSE GOT HUNG OVER ROCK, WENT TO PULL HOSE OVER ROCK, JUMPED OFF BANK, LEFT KNEE TWISTED & POPPED.
OPERATOR WAS HAVING TROUBLE WITH THE STEEL STOPPING UP WHEN HE DROPPED THE HEAD THE STEEL STAYED IN THE HOLE. HE THEN RAISED THE HEAD BACK UP AND WHEN HE DID AS HE HIT THE ROTATION HE DID NOT REALIZE HIS GLOVE WAS CAUGHT BETWEEN THE PINNER HEAD & HIS STEEL CAUSING HIS RT ARM AND HAND TO TWIST AROUND STEEL.
INDIVIDUAL WAS WASHING OUT THE BACK OF A CEMENT TRUCK WHEN HE FELL FROM THE TRUCK BRUISING HIS BACK.
THE INJURED WAS HELPING CARRY A GUARD COVER FOR THE HEADGATE DRIVE WHEN HE STEPPED IN A HOLE AND FELL. THE WEIGHT OF THE GUARD CAME DOWN ON HIS BACK. THE INJURY WAS DIAGNOSED AS A STRAIN REQUR INING REST.
EE HAD JUST FINISHED LOADING BOLTER SUPPLIES ON THE BOLTER IMMEDIATELY PRECEEDING THIS INCIDENT.HE STATED THAT ABOUT A MINUTE AFTER WALKING AWAYFROM THE BOLTER HE EXPERIENCED A SHARP PAIN ON THE LEFT SIDE OF HIS CHEST. HE EXPERIENCED NUMBNESSIN THE FINGERS OF HIS LEFT HAND AND HIS CHEST HURT WHEN HE MOVED HIS ARM. UPON EXAMINATION IT WAS DETERMINED HE STRAINED HIS CHEST.
TWO EMPLOYEES WERE WELDING AT THE SAME TIME ON METAL BOXES. SOMETIME DURING THIS PERIOD ONE EMPLOYEE BURNT HIS EYES.
EMPLOYEE WAS CARRYING ABOUT 30 LOBS OF TOOLS UP A FLIGHT OF STAIRS ABOUT 15 FT IN LENGTH TO GET TO THE BLUNGER CATWALK. HE HANDED THE TOOLS DOWN TO A CO-WORKER INSIDE THE BLUNGER. HE THEN CLIM BED DOWN INTO THE BLUNGER AND BEGAN SETTING UP HIS TOOLS. A SHORT TIME LATER HE FELT PAIN IN HIS CHEST AND LEFT SHOULDER.
THE GATE ON #4 BIN FAILED TO CLOSE. I WENT TO 2ND FLOOR TO CLOSE THE STEER HORN ABOVE THE GATE. MY FINGER GOT SMASHED BY THE COUNTER WEIGHT AND THE ARM ON STEER HORN.

EE AND A CO-WORKER WERE LOADING 10' BY 5"" BELT RAILS INTO A SCOOP BUCKET, EE HAD HIS HAND ON A BELT RAIL IN THE SCOOP BUCKET WHEN THE CO-WORKER TOSSED ANOTHER BELT RAIL INTO THE SCOOP BUCKET, HITTING EE'S RIGHT HAND, FRACTURING HIS RT RING FINGER.
WHILE CLIMBING OVER A PIECE OF MOBILE EQUIPMENT TO RUN JUMPER CABLES, HE FELL OFF CRACKING 2 OF HIS RIBS.
EE REPORTED SERIOUS PAIN IN HIS LOWER BACK. HE SAID THAT HIS LOWER BACK HAS BEEN SLOWLY PROGRESSING IN PAIN OVER THE PAST SEVERAL WEEKS. HE FEELS THAT THIS INJURY IS ASSOCIATED WITH THE LONG H OURS OF WORK. HE COULD NOT ATTRIBUTE THE PAIN TO ONE SPECIFIC INCIDENT.
EE STATES THE MUCKER SHOT WAS OPERATING REMTILY DID NOT STOP WHEN SHE WANTED TO SHE ALSO STARTED SHE RAN TO THE RIGHT OF THE MUCKER AND WAS PINNED AGAINST THE RIB. AND GOLD AS OF THIS DATE IS STILL INVESTING THIS ACCIDENT AND HAS NOT YET DETERMINED THE AU CAUSES FOR THIS ACCIDENT.

WORKING WITH CHAIN HOIST AND THE CHAIN BROKE AND HIT HIM IN THE FACE.
EE WAS PUSHING OVERBURDEN, WHEN HE REACHED THE END OF HIS PUSH HE REVERSED DIRECTION AND RAN OVER A ROCK WHICH JARRED HIM AND CAUSED HIM TO FULL A MUSCLE IN HIS BACK.
EMPLOYEE SYAYED THE FRONT OF THE NOSE ROLLER AREA OF THE HEADRIVE WHEN HE STATED THE SPLICE PULLED HIM INTO THE ROLLER AREA BETWEEN THE BELT AND SCRAPER. HE WAS TRANSPORTED TO THE HOSPITAL WHERE IT WAS DETERMINED THAT HE HAD NO FRACTURESONLY SCRAPES AND BRUISES.
THE INJURED & ANOTHER EE WERE LIFTING A TOP BELT ROLLER & HIS OFF HAND WAS CAUGHT BETWEEN THE MIDDLE & OUTSIDE ROLLER ON HIS SIDE. THE INJURY WAS DIAGNOSED AS A FRACTURE & LACERATION TO HIS LE FT INDEX FINGER.
WHILE MOVING &/OR BENDING TO PICK UP A LARGE ROCK OFF OF A CONVEYOR BELT, EE HEARD A POP IN HIS UPPER BACK. HE DID NOT ACTUALLY PICK UP THE ROCK HE WAS JUST MOVING/BENDING TO REACH FOR ROCK.
EE & 2 CO-WORKERS WERE WORKING ON THE 005-0 SECTION, HANGING MINER CABLE. EE'S ARMS WERE ABOVE HIS HEAD WHEN HE PLACED MINER CABLE AND WATER LINE INTO THE CABLE HANGER. EE IMMEDIATELY FELT PAI N IN HIS MID BACK. HE WAS TAKEN TO WILLIAMSON MEMORIAL HOSPITAL WHERE HE WAS DIAGNOSED WITH A HERNIATED DISC AT L4.
EMPLOYEE WAS HELPING PULL CABLE ON DRAGLINE AND IT SPRUNG BACK HITTING HIS LEFT HAND.
EE WAS ASSIGNED TO A TASK REQUIRING HIM TO PERIODICALLY WORK IN A CROUCHED POSITION OVER SEVERALDAYS. HE DEVELOPED PAIN IN HIS LOWER BACK THAT COULD HAVE POSSIBLY BEEN RELATED TO THIS TASK. THIS RESULTED IN LIGHT DUTY.
EE WAS CHECKING #1 CLINKER COOLER DRIVE MOTOR. IT HAD STALLED BUT WAS SHOWING A RUN FROM THE OPERATOR'S PANEL. HE REMOVED THE GUARD FROM THE MOTTOR-GEAR REDUCTION COUPLING TO SEE IF HE COULD S PIN IT BY HAND. FAILING THIS, HE TOLD THE OPERATOR TO SHUT THE DRIVE OFF. THE BACKLASH CAUSED THE COUPLING TO SPIN IN REVERSE, SNAGGING HIS GLOVE, PULLING HIS ARM AROUND THE SHAFT OF THE MOTO
EE WAS PULLING ON MINER CABLE WHEN HE FELT INTENSE PAIN IN HIS LOWER BACK.
EMPLOYEE STATED THAT WHILE EXITING THE TRUCK, HE FORGOT SOMETHING IN THE CAB AND STARTED BACK UP THE LADDER. HE LOST HIS FOOTING AND FELL FROM THE TOP RUNG OF THE LADDER TO THE GROUND.
EE WAS PUTTING OIL INTO A CAT DION BULLDOZER WHEN HE LOST HIS BALANCE AND FELL OFF DOZER AND SPRAINED HIS ANKLE.
EE WAS LOADING A TRUCK. SAID HE HIT A BUMP AND FELT PAIN IN HIS BACK. HE SAT DOWN, ATE LUNCH, AND ERNT TO 6TH FLOOR TO HELP CHANGE A CYCLONE PIPE. WHILE WALKING ACCROSS THE FLOOR HIS RIGHT LEG & FOOT GAVE OUT.
EMPLOYEE SLIPPED ON THE CATWALK OF THE SILO TRANSFER BELT, FELL AND HURT HER SHOULDER.
WHILE WELDING, A EMPLOYEE RECEIVED A FLASH BURN TO HIS EYES.
INJURED WAS AT THE POWERCENTER. HE BENT OVER TO PICK UP A PIECE OF TRASH AND FELT PAIN IN HIS KNEE. HE WAS TAKEN TO HOSPITAL AND DIAGNOSED WITH OUTSIDE LIGAMENT TORN INTO AND INSIDE LIGAMENT TORN ON RIGHT KNEE. HE HAS A HISTORY OF REPEATEDINJURIES IN MEDICAL HISTORY TO RIGHT KNEE.
THE EE WAS PUTTING HOSES ON SHIELD WHEN HE FELT A PAIN IN HIS LOWER BACK.
EE WORKED ALL DAY 9-10-2002. HE CALLED IN A 2255 ON 9-10-02 AND STATED HE WOULD BE OFF WORK UNTIL 9-16-02. NO IMMEDIATE ACCIDENT REPORT WAS COMPLETED.
EMPLOYEE WAS HOLDING THE HYDRAULIC DRILL WITH HIS LEFT HAND & REACHING DOWN TO THE MINE FLOOR TO PICK UP A WRENCH WITH HIS RIGHT HAND & FELT A CATCH IN HIS MID TO LOWER BACK AREA.
EE WAS WALKING TOWARDS BELT CROSSOVER WHEN HE SLIPPED IN MUD FALLING ONTO MOVING BELT. INJURIES WERE BRUISED RIBS AND A PULLED MUSCLE IN LOWER BACK.
