A PLC Emergency Stop Fault on the Koepe Drive would not clear after power tripped. Power was lost to the Koepe Hoist while three employees were underground attempting to restart the main fan. The employees safely evacuated the mine via the production hoist. The hoist was put back into service at 6:00 PM 10/31/2012. There is no damage or injuries to report.

During the weekly shaft inspection of the production hoist power was lost to the mine at approximately 4:45 AM. An off-site power line was knocked down during a storm resulting in the mine losing power. Two Miners were in the shaft performing the inspection during the outage. The local power company restored power at 8:30 AM and the miners safely exited the mine with no injuries.
An unintentional roof fall occurred on the 2 North 1, Entry 2 Crosscut 21 thru 22. The roof fall measured approximately 80' in length, 17.5' in width, and 6' in thickness. A 103 (k) Order (8448216), was issued October 30, 2012 by MSHA Inspector, Glenn E. Fishback.
Employee was inside service elevator when it became stuck between the 5th and 6th floors. This condition lasted over 30 minutes until the elevator service company responded and activated the elevator and corrected the stuck button.
An unintentional roof fall occurred on the 4th North belt line @ the 218 crosscut (station 173+71) Entry #5. The fall did not affect ventilation, passage of men or cause personal injury. The fall did cause damage to the belt line. The area will be cleaned up and arched. The area had been bolted with 6 foot roof bolts. The fall measured 35'x 20'x 15' high.
An unintentional roof fall occurred on the 1 North 1 Right Return, Entry 8 Crosscut 41 thru 42. The roof fall measured approximately 60' in length, 17.5' in width, and 8' in thickness. A 103 (k) Order (8448217), was issued October 30, 2012, by MSHA Inspector, Glenn E. Fishback.
THE MAIN #1 HOIST (NORDBERG 7030-1084) WAS TAKEN OUT OF SERVICE 10-29-2012 AT 7:40AM. SERVICE WORK ON THE MG SET WAS PERFORMED OVER THE WEEKEND. NOISE AND LIGHT SPARKING WAS COMING FROM THE MOTOR. INDEPENDENT ELECTRIC WAS CALLED. IT WAS DETERMINED THAT 2 WIRES HAD BEEN CROSSED. THE WIRES WERE DISCONNECTED AND HOOKED UP PROPERLY. HOIST WAS PLACED BACK INTO SERVICE AT 8:20AM.
A fire occurred in the West Coal plant under silo #2. Appears that spontanous combustion of coal took place in the silo feeder and fell from the center feeder above the belt onto the belt and igniting the belt. The plant had not been operated for 3 days prior to the fire.
# 2 hoist tripped out on limit switch.
A weekly Examiner identified a roof fall in the 8D section of the mines measuring 75X 20 X 5 with a distance of 5,978 from the face. This fall does not interfere with airways or travel ways.
There was a roof fall in the #2 Entry of 4 South Mains at 13 1/2 wall, which did not affect ventilation. The fall measured approximately 100' long, 15'w and 10' high. *No injuries or equipment was damaged as a result of this incident.
A roof fall was determined to be above anchorage at main line 15 belt, #26 crosscut. No injuries, travel ventilation are not affected. Dimensions are roughly 60' long x 16' wide x 12' high.
The hoist car at Roadfork #51 mine was taken out of service for a period of time that exceeded 30 minutes. When x-rayed it was determined that the hoist cable had deterioration that caused it to be changed out with a new cable.
An unintentional roof fall occurred at the Galatia Mine New Era Portal at the 1610' West mark between #2 and #3 Entries, in the 10W Tailgate. The fall was approximately 25 feet long, approximately 18 feet wide and approximately 8 feet thick. The fall area will be re-supported according to the MSHA approved Re-support Plan. Fall #29B
A roof fall at Arabian Mains at spad 4108 and 90feet outby spad 4094. Measuring 20 foot in length 20 feet wide and 6 foot high. 48 in glue was used in roof support. This area will need to be cleaned up and overcast installed.
On October 24, 2012, San Juan Mine experienced a roof fall in the Longwall 301 panel Headgate entry at approximately 14.5 cross cut in the belt entry. The fall is approximately 7 ft high, 18 ft wide and approximately 45 ft long. No injuries or equipment damage occurred.
Fire in the bus bar raceway above the ""West 10x8 Slurry Pump..."" cabinet in the MCC in the unoccupied, idle Fine Crusher. Spare parts that were stored nearby were ignited. The raceway, bus bars, bus bar supports and ten ft. of cable in the E11 tunnel were damaged. The cause of the fire is thought to be an animal contacting the bus bars or power cable failure.
Roof fall at x-cut 71 in the number 2 entry (belt entry) of Headgate #2 section.
An unintentional roof fall occurred at #123xc, #4 Entry, of the Cedar Grove Mains. The dimension of the fall is approximately: 100' (L) x 19' (W) x 12' (H). The area has been dangered off, and a clean-up plan will be submitted.
Stress Zone and poor roof conditions. Roof fall found by MSHA Inspector and Section Foreman.
A roof fall measuring approximately 8' x 20' x 70' feet fell in the No. 9 to No. 10 crosscut. One crosscut inby spad No. 752.
On return of day shift, the heading was discovered by inspection to have had a roof fall.
A roof fall was discovered in the 5 entry 1 xcut inby spad 608. The fall is sitting on cribs and not broken up about 3'above the mine floor. The area is apx.40'Wx20'Lx7'H.The fall has no effect on miners, vent. or equipment. Supplemental roof support consisting of cribs and beams has been installed. An investigation is underway to determine if cleanup of the fall will be required.
Roof fall occurred at or above the anchorage point in 17 West J X 80 South.
The Crabapple elevator was inoperable for an extended period of time. No injuries were a result of this incident. The door switch needed replaced.
An unintentional roof fall was discovered at approx 5:50am. In the number 5 entry on the Grassy Creek Mains at break 54. The fall extended to the number 6 entry intersection by excluded the number 6 entry. All approaches to the fall were timbered off. The fall did not interrupt ventilation but will be cleaned up. The fall is approx 110' long, 20' wide and 12'high.
A roof fall was discovered on U-section at Survey station 9755. The fall area was approximately 6' in height, 20' in width and 20 plus feet in length. The fall area was mined in 10/1997 and was supported with point anchor bolt system. The fall area was dangered off to prevent access.
A roof fall above anchorage was found at main line 17 belt at #16 crosscut. No injuries, travel and ventilation are not effected. Dimensions are roughly 30' long x 12' high x 16' wide.
An unplanned roof fall occurred on #3 section panel 800' from the face @ spad #8136. Area was originally mine in Sept 2012 and was supported with 8' cable bolts and 5' tourque tensions. Fall measured 20'L x 18'W x 10'Thick. No injuries or equipment involved. Did not affect passage of persons or ventilation. State and Federal agencies notified immediately.
A plant maintenance inspector was inspecting the wear liners in a ball mill when his foot sunk into a soft spot of mud and he was unable to get it unstuck without assistance. No injury or exposure occurred to the individual however this was called in out of an abundance of caution.
Men were fixing to get on cage when they noticed a distorted Hoist Rope. The Cage was shut down until inspection and repairs can be made.
A fall of the mine roof was discovered at 85 brk on 15 HG. The fall was located in the c.c. between #2 & #3 entry. The fall meas. approx. 20' W. X 60' L. X 6' Thick. This roof f fall does not affect ventilation or travel in the area.
An unusually strong storm with high winds caused a regional power outage which resulted in the loss of electrical power for most of the Silver valley including surface and underground at the Galena and Coeur mine. This outage effected the hoists at both mine properties.
Failure of the #1 hoist plc processor prevented the mine from raising and lowering personnel with the #1 hoist. The mine still maintained two means of egress with the #2 and #3 hoists.

A roof Fall was discovered in the #1 entry less than 1 cross cut outby spad 615. The fall was approximately 40'long X 20'wide X 7'high. The fall was cleaned up with the continuous miner and re-supported with cribs, beams and roof bolts. The fall did not have any affect on miners, equipment nor ventilation.
While traveling the intake an examiner identified a roof fall in the #6 entry inby spad #1195. The fall measured approx. 20'W X 60'L X 4'-5'H and was bolted with 4' fully grouted resin bolts. The area was mined approx. 8/2/12 the area will not be cleaned up and the area has been dangered off and timbers set.
A severe storm system caused a tree to fall across power transmission lines that service the Coeur mine. This caused a power outage to all surface and underground areas of the Coeur mine including the hoist.
At approximately 6:40 pm on Tuesday Oct. 16, 2012 a possible ignition occurred at the face in Unit 1. The unit was immediately shut down, power removed and the proper persons notified. The scene was preserved for the company and agencies to conduct an investigation.
Coal barn attendant reported the smell of coal burning at 11:20 am. He inspect coal storage barn, noticed area appeared to be smoldering. He attempted to extinguish, air monitor alarmed. Operator notified supervisor 12:05pm. At 12:10pm, supervisor restricted entry. Contacted Ishpeming Township Fire Dept, ITFD extinguished the smoldering coal. ITFD off-site 2:45 pm.
A 657 scraper operator was on his third round of topsoil placement for the day when the fill blade operator notified him of flames coming from his rear engine. The scraper operator stopped and contacted supervision. The fire department was called immediately. The fire department arrived on scene and proceeded to put out the flames utilizing foam.
During his fire boss run the examiner identified a roof fall in the #3 neutral entry outby spad#1107. The fall measured approx.20'WX60'LX4'-5'Hthe area was mined approx.5/30/12. The area was supported with 4'fully grouted resin bolts the fall will not be cleaned up and timbers have been set and the area dangered off.
The roof in the tailgate entry for the longwall had begun to deteriorate on the midnight shift. The travelway would have required supplemental support - posts or jacks at that time. During the course of mining on the dayshift, deterioration of the roof continued until the tailgate travelway was deemed impassable. District 3 Ventilation Supervisor was notified at approx. 3:20 PM.
On 10-14-12 at 9:23 p.m. the Elevator went down. There was no one trapped. There was an electrical connection causing the Bodie Brake to read bad. Shindler Elevator Service was called to the mine and repaired the connection. The Elevator was back in service at 12:50 a.m.
The Crabapple elevator was inoperable for an extended period of time. No persons entrapped nor injured as a result of this event.
An unintentional roof fall occurred on the 1C, Entry 6 Crosscut 6. The roof fall measured approximately 25' in length, 18' in width, and 10' in thickness. A 103 (k) Order (8446937), was issued October 14, 2012, by MSHA Inspector.
On 10-14-12 at 6:28 a.m. the Elevator went down. There was no one trapped. The Bodie Brake malfunctioned and would not release. Shindler Elevator Service was called to the mine and replaced the part. The Elevator was back in service at 6:15 p.m. 10-14-12.
Cardboard boxes containing items for storage were put on a shelf below a thermostat controlled heater in a storage conex. The heater was not on when the boxes were placed, but turned on when the temperatures changed. The cardboard became hot and caught on fire. The fire was contained to the front of the conex and was extinguished upon the arrival of fire-fighting equipment.
At approx. 2:05 p.m. a roof fall occurred in the D entry of the 2 right section. The roof fall was approx. 20' x 20' and the height unknown. There were no injuries. The roof fall was dangered off on all sides.
Roof fall occurred in XC13 on the 1st main W off main N between entry 7 and entry 8. The fall is 17,000' outby the loading point. It is between two overcasts providing a limited intake aircourse to two seals. It is approximately 19.5'Wx75""Lx8'H. No injuries/equipment involved. 1 corner of 1 overcast was damaged which allowed limited intake air to cross with Unit 5 intake air.
The Dry Ridge elevator stopped mid way in the shaft with 2 persons on board.
Unplanned roof fall occurred at #18 Break on Old Belt, #1 Entry in intersection. Fall was approx. 20' W. x 20' L. x 8' H. no injuries, entrapment or Eq. involved. No escape way, ventilation or travelway affected.
An unplanned roof fall occurred two (2) cross-cuts outby the abandoned 4 left panel off 3 west mains in the #3 return entry at survey station #3353. The roof fall is approx 20ft long x 20ft wide x 8ft-10ft thick. Ventilation has not been affected. roof fall area will be dangered off and blocked off to prevent access. There were no injuries associated with the roof fall.
A roof fall 20'W. x 40'L. X 16'H. occurred at approx 9:32 a.m. on the 2 right section causing the entrapment of two miners. The roof fall was in the E. entry at spad#21+35. The miners were rescued at 6:56 p.m. with no injuries. The miners were bolting the x-cut E to F.
The blake ridge elevator stopped with 2 men aboard. the 2 men were removed from the elevator. Upon investigation a broken ""Governor safety latch catch arm"" was discovered. The entire governor wheel assembly was replace. the elevator was tested with MSHA & State inspectors before it was returned to service.
Unplanned roof fall occurred #7 entry at Spad 2121 and extending inby 1 crosscut. Fall was approx. 19' W. x 80' L. x 10' H. No injuries, entrapment or Eq. involved. No escape way, ventilation or travel way affected.
Roof fall in the intersection of entry#7 and XC11 on the Main E. travel road The fall is 11,860' outby the loading point.Meas.25'L.X 35' W.X 10'H.There were no injuries and no interruption to ventilation. Travel road was blocked and the secondary escapeway lifeline was re-routed. The fall will be cleaned up and re-hab'd. Personnel have been notified and the changes made on the map.
Found an unintentional roof fall on the #23 Beltline. The fall was 120' long by 20' wide, and 12' thick
15A Longwall Section - It was reported that an alleged ignition occurred at 161 shield on the longwall face.
An unintentional roof fall occurred at the 36+00' rooms at the 60'cross cut in room # 4 on unit #3.
At approximately 11:15 pm 10/7/2012 a roof fall was discovered on the 4th left a panel at cross cut 3 in #3 entry (belt entry). The fall is appx:20'x 56'x 4' to 5'high.
A roof fall above anchorage was discovered on main line #7 belt at #9 crosscut. The # fall is 55'long, 12'high, 16'wide no injuries occurred. Travel was effected ventilation was not effected.
An unintentional roof fall occurred at crosscut 270 in the Main North Intake in the #3 entry. The fall had been roof bolted with 6 foot fully grouted roof bolts. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 30 feet long x 16 feet wide x 3 foot thick.
At approximately 03:00am a fall of ground occurred near the face at the right spring line of the 4455-909 BC stope access which was being shotcreted in preparation for bolting. The boom of USM006 was covered in muck. At the same time, the left rib on the 4490-909 MC stope access subsided. The left rib was in the process of being bolted. No miners were injured.
An unintentional roof fall occurred at the New Future Mine 4W Headgate Entry #3, 5627W, Crosscut 41. The fall was approximately 25 feet in length, approximately 25 feet in width and approximately 20 feet thick. The fall did not affect ventilation. The fall will be cleaned up. Fall #29E
Damage to the topside ore bin vibratory feeder prevented the mine from emptying the ore bin (which was full) and the #1 hoist south skip could not be unloaded. This prevented the mine from raising and lowering personnel with the #1 hoist. The mine still maintained two means of egress with the #2 and #3 hoists.
The door locks on the elevator malfunctioned
A roof fall occurred on # 16 butt section, in the # 2 entry working face. No injuries occurred, and no equipment damage was incurred. The roof and ribs were supported to the roof control plan. The mine roof immediately preceding this fall had supplemental roof support in the form of roof screen,& v-beams & jacks.
After the yearly examination and new terminations of the slopes, the hoist was lowered to the bottom and while being raised back to the surface, the capsule become misaligned causing a guide to be bent and slide to be broken.
The fall was in the old works not affecting travel or ventilation. Return Rooms of 2 south mains #1 and #2 room at spad #3422 outby MP-6 80' wide x 60' long x 6' high.
# 5 hoist was lowering workers to 850 level, when left brake on hoist drum slowly engaged. Workers were stuck 9 feet above 700 landing. Low pressure on left brake hydraulic system caused brake counterweight to drop and engage brake. Workers communicated to hoistman and climbed down to lower cage and exited to 700 landing at 6:56 AM.
Foreman started a D11 dozer prior to the start of the shift to warm up the equipment for the operator. When he returned with the fueler, the equipment was in flames.
A roof fall was discovered on the above date and time. It was located in the #6 entry at survey station #2475 which is in the intake air course. The fall was 35 feet in length 20 feet wide and 42 inches in thickness. The time and date the fall happened is unknown.
Roof fall on INE mains #3 entry outby spad #12032 for 40'length above belt and the width of the 3-2 crosscut for 15'-20'.

3 EE's riding service hoist cage when hoist power dropped out. Hoistman could not reset. 3:11 pm Mine Supt notified; 3:22 MSHA notified. Electrician from off-site called. EE's lowered to station under gravity/friction procedures. > 30 minutes elapsed before EE's reached station. Power reset and functional 4:29 pm.
An entrapment occurred on the Metz elevator involving 16 people for 92 minutes.
The ventilation system (dampers) for the cooling system for the west hoist motor malfunctioned causing the hoist motor to overheat.
An unintentional ignition occurred on the number 1 Unit face, between Entries 4 and 5. A 103 (k) Order (8447221) was issued October 1, 2012, by MSHA Inspector.
Forklift turned over - The miner was operating a forklift, traveling on a decline toward the mine entrance, when the forklift went out of control. The forklift struck a concrete support for the belt conveyor and overturned, killing him.
Employee was sitting in a Cat 988 with the window open. When he went to shut the window, the wind caught it, slammed shut, and cut the tip of his right little finger off.
Miner was conducting repairs on a screw conveyor. Miner failed to lock out tag out the screw conveyor. The miner indicated that the screw conveyor inadvertently started. Miner received severe injuries to his right hand.
Employee states that while dusting drift at mannings branch and 3A head, I picked up a bag of rock dust and felt something pull around my groin area. It then started to swell.
Employee was unloading new rail ties from flat. as he rolled the top tie to the edge of flat, it caught the edge of the tie under it, causing it to flip & land on top of his fingers. went to medic. we took to ER. x-rays confirmed fracture in finger tips #3 & #4 - right hand. restrictions were issued.
Skid Steer operator, while backing up with a functional alarm sounding, struck the plant operator, resulting in leg and ankle/foot injuries to the plant operator.
Moving a 3' x 5' x 3/8"" piece of steel, steel rotated pinching hand.
While driving scoop, EE hit a hole or a rock on roadway causing him to be bounced up from the seat then back down straining his lower back.
Employee was pounding a bolt thru plastic liner. A piece of plastic went into his eye. He was wearing Safety Glasses.
Injured placed hand between counterbalance flywheel and side plate of ""B"" Raw Coal Screen to check temperature of bearing seal, when fellow employee pushed start button to jog machine causing injury.
Employee was standing in crosscut waiting for a scoop operator to return. While waiting a piece of rock measuring 24""x4"" fell striking employee and knocking him to ground. When the employee braced himself falling to the ground he jammed/sprained his left wrist.
Employee was loading roof mesh and stepped off mesh onto crib block rolling his right ankle.
Employee told me at the end of the shift, 1:30 A.M., that his back was hurting. He went on to say that earlier in the shift, the loader operator had jarred his rock truck with a rock while loading the truck. He said he may go to the doctor to get checked out some time the next morning.
Dewatering personnel was on the ground near the area where a decommission electric cable was being moved when the cable stuck him in the lower leg.
EE was getting out of the Scoop when he felt pain in back-EE refused ambulance and drove himself to hospital
While employee was tearing down Hatachi 1200 Hoe , He was using a wrench and his right arm locked into a bent position. He used his left arm and straighten out the right arm. Finished out the day working. Worked out the balance of the week and went to Occupational Health that Friday 11/02/2012
Employee was picking up 50# AP sand bags and went to put it on a pallet when he felt his shoulder R pop out and in and now he is having pain in his shoulder. Diagnosis: Right Shoulder Stain.
EE was working behind the cab of a 773, reaching into the compartment in a bent postion to trouble shoot air brake system. When he came down from the truck he felt pain in his lower back. (Eq.# 1384)
Stated: Employee was climbing ladder to secure when the bottom of the ladder slipped out. Employee fell on right shoulder. He continued to work. He sought medical evaluation on 11/21/12, was diagnosed with arthritis and slight rotator cuff tear. He continued to work. He underwent arthroscopic repair on 01/16/2013 and is now off work.
Employee was dumping trash using a self dumping steel forklift hopper, released lock lever and the hopper bounced back smash his finger.
The employee was constructing a block brattice. As he was moving a block he tripped over a steel band and strained his back. He went to the doctor on 11/9/2012 and was put on restricted duty.
EE slipped on 777 Cat Haul Truck. He lost his footing and struck his knee.
Doing maintenance on Thor Stacker conveyor, putting wear iron in receiving box using torch and welder. He slipped and fell because of rainy conditions and smashed and cut his middle and pointer fingers on his left hand.
EE was walking around x-cut 11, 5 entry of North Main to pick up floats to pump water. He stepped on 4"" pipe and slipped and fell on left shoulder when he put his arm out resulting in shoulder strain.
Employee was lifting a roller when he felt his back pop. Employee went to Hospital sustaining a strained back and was ordered to remain off work for an uncertain period of time.
Employee was in squatting position and knee gave out when standing up.
Pulling on super post to get it un stuck from scoop bucket and felt something in my back pull.
Employee was stepping through man door stepped down wrong and twisted left foot. He has a fracture to his left pinky toe.
EE was trying to move a 5 gal bucket to pick up a dropped solaris spotter. While attempting to move the bucket EE stated that he had hurt his neck-back and had pain in bolt legs. EE was transported to Logan Hospital via Ambulance
Employee was replacing cutter head on continuous miner and was attempting to replace top pin of the cutter jack. Coworker raised jack with slate bar in upward motion. Employee was using his hands to pull jack in place to replace pin. The slate bar slipped allowing cutter jack to smash Employee's left ring finger against frame of pan.
EE was helping to take down a joint of ventilation tubing. The operator on the other end of the tube dropped his end hyper extending EE's fingers.
During a unit move the mechanic was repair a miner cable. As he was re-installing the receptacle on the cable the unit low trac was pulling slack on another cable. As the low trac went by the mechanic the operator didn't see him and the low trac ran over his foot.
Employee states that while dusting the track entry on 1 belt he was walking thru the overcast when he raised up and struck his head on a metal beam. Employee did not report this incident until 10/28/12 to a man bar of mgt at home. A incident report was filled out on the 27 when mgt returned to work.
Employee told Foreman that he hurt his back, When asked by the Foreman, did he hurt it at work, the employee stated that he did not hurt it at work and he had turned it in on his health insurance. We later received a letter from a lawyer stating that the employee hurt his back at work.
EE was pulling shields on the 18-W longwall face. When he stepped up on the pontoon of 49 shield with the left foot and the right foot under the pontoon of the shield that had not finished it's setting cycle. EE received a fracture to the right big toe and the next toe also.
Employee turned the A/C unit on and an unidentified object flew into the employees eye.
Operator was tramming a track drill on a snow covered bench when he hit a loader tire rut approximately 8"" deep. When the drill came out of the rut the front tracks went up into the air and slammed to the ground jarring the operator while he sat in his seat.
EE was stepping through the threshold of a doorway and caught the toe of his right boot on the door jam causing him to trip and caught himself and felt pain in his right knee. EE stated missing work on 1/21/13 due to having surgery to repair a torn meniscus. EE is off until further notice.
Employee stated he was taking a hubcap off a Komatsu 630E and slipped and fell to the ground injuring his left hip and right knee. Employee's family physician referred him to St. Marys Occupational Health Center. Dr. diagnosed him with a sciatic nerve issue on his left hip and sprained right knee and took him off work.
EE was walking up a stair case and stepped on the 11 tread and it fell out from under his right foot on 10-25-12. He has been treating with a Physician and was taken off of work on 1-10-13.
Loading railing, install one side and when you lift the other side falls catching two finger of the right hand against the concrete.
Injured employee had backed the continuous miner into the intersection, shut miner off and was in the process of moving miner cable away from the side of the pan at the back of the ripper head, when a piece of material slid off the continuous miner striking his left foot resulting in a fracture.
Employee had his hand laying on the tray of bolt machine watching the other operator set his bolt, when a rock fell hitting his left hand. He was taken to the ER and received an x-ray that was negative. He received four sutures in top of his left hand.
Employee was sitting on his stinger ride on #1 unit 2 x-cuts out by the face in #5 entry when a sharp piece of draw rock fell out between the pins and struck him on his left knee causing a contusion.
Miner was operating a 777 cat haul truck and sprained his back. He felt that the suspension on the seat was too stiff and the bouncing was too hard on his back.
Employee was installing a rib bolt when the plate slid down the bolt and struck him in the left hand.
Employee was operating a shuttle car, he hit a hole in the roadway bounced him into canopy hitting his head. He received an x-ray at ER that was negative.
Employee stated that he was stepping over a retaining wall when he twisted his knee. An MRI showed a torn meniscus. His first lost work day did not occur until 10/29/2012.
Employee got dirt particles in his right eye causing a cornea ulcer. He had his safety glasses on.
Employee caught finger between mine roof and roof bolt plate causing a laceration.
On 10/24/12 EE strained his rgt. side back and hip area while installing a water line. EE declined medical attention at that time. EE reports that on 11/28/12 he further aggravated the injury while performing the same task. He worked through the day. Worked 11/29/12 then sought medical treatment that evening in the emergency room and was placed out of work.
The employee scraped the face with an LHD bucket preparing the face for drilling. He exited the LHD to analyze the face for further scraping when a rock fell off the face striking his right foot beyond his boot metatarsal guard causing a fracture.
He was lifting a stone when it slipped and pinched his hand between the stone he was lifting and another below it.
STARTED TO PUT HIS LEFT KNEE UP ON THE BACK OF THE SCOOP BUCKET AND HIS RT. KNEE POPPED 2 TIMES THEN HE FELL
While attempting to mount the D9H bulldozer, employee turned around on the ground and felt a pain in his right leg. Employee was taken to the hospital for observation.
Employee incurred a broken nose after being struck by a 2"" waterline. The employee was moving the waterline and other utilities to prepare the back to be mechanically scaled. While the employee was in the process of strapping the waterline to a roof bolt plate the waterline popped out of the cable handler (U Shaped bracket in a high lift) and struck the employee in the face.
The employee fainted and fell to the ground while walking from his lunch break area to his work area. He landed on his face and sustained abrasions to his face, knees, and elbows. His tongue was also lacerated and required 4 stitches. 9-1-1 was called and he was treated and released at the ER.
The employee stated that he and two other employees were pulling a 2"" waterline with the scoop. They were utilizing a 1/4"" cable to pull the waterline and the cable broke and recoiled back striking the employee. When the cable struck the employee, he fell into the belt structure striking his chin and received 7 stitches from the injury.
Injured employee was in the process of installing permanent roof support. He had completed the drilling process and was reaching for the bolt that was laying on the mine floor next to drill pot when a piece of material fell striking his left index finger resulting in a laceration and fracture.
EE WAS TRAMMING THE SHUTTLE CAR, WHEN HE HIT A HOLE AND BOUNCED HIM INTO THE TOP OF SHUTTLE CAR CANOPY. THUS RESULTING IN A NECK INJURY.
Injured employee was in the process of installing permanent roof support. He had placed the end of roof bolt into the mine roof and was in the process of straightening the bolt when he felt a pull in his lower back. He continued to work until 10/30/12 when he was seen by a local physician and removed from work.
Employee tripped over a stack of blocks and fractured his knee.
Employee caught toe on metal plate laying on the floor as he turned to walk and felt pain in his right knee. Case was not reportable until Dec.20 when employee had surgery.
Employee was dismounting the front end loader when he stumbled striking his right wrist on the ground resulting in a fracture
Employee was climbing up and down ladders all night working on the 944 loader. When he descended a ladder he felt a pop in his knee. Employee went in for surgery on knee 12/20/2012, becoming a lost time injury.
Employee was changing a leaking valve and was sprayed with acid mid after bumping a different valve on a separate line. Mist sprayed between overlaps in employee's PPE causing chemical burns to his waist and wrist.
The employee was adjusting a spring on a screen. While trying to get it into position the spring shifted cutting off the tip of his thumb between the spring and the cup. He was taken by us to hosiptal where they looked it over and wrapped it and said it will heal on its own, no stitches required.
Employee stated that he was leaning timbers against the rib to prepare to install them on the mine roof. One of the timbers slid off of the rib and struck him in the side and ribs. The employee was in an automotive accident prior to the incident and had broken his ribs. This incident re-injured his ribs. Employee began missing work on 10-25-2012
Replacing belt on vibrator - pinched hand between belt & pulley
Employee was stacking belt structure in supply yard, there was a ram jack that was entangled in structure when trying to remove the ram jack out shoulder popped. PPE was not a factor, mining equip/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor. This miner missed work for this injury as of 11/18/2012. Lost Time Injury.
while using high volume fire hose to wash ASH Truck window, employee stepped forward on a mud and strained right knee.
Employee was loading steel into a sea container while load was being slid into position inside the container it pinched employee's left lower leg against the side of the container.
EE was loading shot on 11AY with the Ampho Hose and a small piece of rock came off the face at his back and struck him on the neck. Initially he didn't feel he had any injury, but the next day he was sore. We took him to the doctor for review and he was diagnosed with a neck strain.
Worker was working in a group of 3 individuals using a vaccum truck to remove debris from around a conveyor belt on an elevated platform. As he was pulling the 4 inches of hose, slipped backwards falling down 3 stairs striking his tailbone. The injured worker indicated he was not paying attention to his surroundings.

Repairman was at section power center that is located between #14 and #15 entry. Botton was cut in #14 entry leaving off set in bottom as he was leaving foot slipped and slid under 3 wheel ride that was parked resulting in twisting of knee.
Employee was using a strap hooked to a man trip to pull water line. The strap broke striking employee on the neck and face causing a contusion to his neck and broke his front tooth.
EE was working line curtain when he twisted his knee and felt pain. The EE never missed work until 1/30/13.
Clear night, no wind. Employee was welding when a hot piece of slag fell and burned threw the tongue of his boot burning into his foot.

This action caused the scaffold to move near the edge of the concrete stoop it was on. EE lost his balance during this move, causing him to fall. A spotter was with him but unable to react quickly enough.

Employee was pulling hoses through brattice and a part of the plate that the sheave wheel was connected to broke off hitting employee in the back of leg, right calf, causing a contusion and bruising and started loosing time 10/19/12.
Employee was sitting on a tool box in the dinner hole when a piece of coal rolled out from the rib. As the employee ran from the area he stepped in a hole and twisted his left knee.

MMU001 No. 3 entry SS#3845. Employee had drill steel leaning against tray while installing a belt hanger, when he lowered the drill head the drill steel fell between the drill head and bottom causing the drill steel to bend and releasing striking employee on the right elbow causing a fracture.
Employee was walking down the haul road when his left knee popped. After a subsequent doctors visit on 10/19, an MRI was ordered for 10/22/12. He was taken off work on 10/23/12. MRI revealed degenerative disease and abnormal softness of cartilage.
Injured employee was in the process of installing cutting edges on a D11R dozer, while lifting the cutting edge with a 6ft. nylon strap rated for 1,550 lbs, the strap broke causing the cutting edge to fall on the employees right foot. He was transported to the ER and diagnosed with multiple foot fractures and required surgery the following day. He has been excused from work.
Coal sample operator slipped while exiting the auger control room and strained his lower back.
EE was helping the continuous miner operator move the miner from the #5 entry to the #6 entry the miner operator stopped the miner to hook up a slack rope the miner operator hooked the rope to the boom of the miner went to turn around and stumbled and somehow hit the boom swing lever and somehow EE's hand was where the boom swung into the rib smashing his left hand.
dismounting coal loader using 3 points of contact, missed or slipped on last step and fell to ground landing on left elbow, employee was transported to Summersville Regional Hospital, x-rays reveled two broken bones in lower arm
Employee was carrying a 6 foot timber with another employee when he wrenched his lower back. The top was low in the area. Employee continued to work until 11/8/12 when the Doctor took him off work and referred him to a neurosurgeon.
Employee lifted a compressed gas cylinder, started to walk, stumbled/slipped, and strained his right shoulder. Employee finished the work shift, and continued working with no time lost, until January 18th 2013. On January 18 2013 the employee stopped working to acquire a surgery to repair his right shoulder rotator cuff.
Employee was bending the keeper latch (on a mucker) that holds the air cleaner housing cover in place, while doing so with a pair of pliers, the pliers slipped off the latch resulting in the top of his left hand being struck against the hook on the end of the latch resulting in laceration.

Employee was building rotex screens and he told his supervisor that his hip was sore. He went home & called the supervisor a few hrs later as said his hip was still sore and that he would ice it down.
Maintenance mechanics were removing boom point sheave from the boom point saddle with a crane. As sheave was being guided out the employee's right arm was caught between sheave and boom structure.
Employee was loading top belt rollers into the scoop bucket and heard a pop in his right shoulder, followed by pain. This is an old injury that he has been having issues with.
A BMX employee was preparing to exit #34 Brookville mantrip that was parked at 59 Cross-Cut on the East Mains haulage. As he began to exit the operator's compartment his left heel caught a 1"" lip that was located on the first step out of the operator's compartment. He twisted his left foot during his dismount.
On 10/15/2012 the miner was handling screen wire mesh when he felt his shoulder pop. He went off work on 6/16/2014 for surgery to his left shoulder.
While trying to loosen the bridge pin on the miner, the ratchet slipped and the employee fell against the conveyor striking his right shoulder causing a shoulder strain. The employee continued to work until 2/11/13. He had surgery on 2/12/13. This resulted in a lost time accident.
EE was standing on top of a conveyor belt and reaching for a cable when his foot slipped causing pain in his leg.
Employee was roof bolting when letting down the canopy it caught his left hand on bolts in his tray with the canopy it mashed his middle finger and index finger on his left hand.
While driving scoop, EE hit a hole in the roadway causing him to be bounced from the seat then back down straining his lower back.
While using a cutting torch a piece of hot slag fell onto the acetylene hose and burned a hole through it causing it to blow out. Employee used his right hand to push the hose away from him causing him to receive a burn on his right middle finger and hand.
Employee was moving cribbing blocks and mashed his small finger on his right hand.
While walking down a spiral stair case injured worker felt a pop in his right knee.
He dismounted a LHD and slipped on a rock, hyper-extending his knee. The accident occurred at approximately 0800 however the employee continued to work on the injury until he reported it at 1430. This potentially aggravated the injury beyond the treatment of basic first aid.
Employee was pulling on a mine de-watering pump that was stuck in the mud, causing pain in the employee's abdomen.
Employee tripped on a grade and fell on his knee. The results of the MRI along with a medical opinion dictated on 10/31/2012. The diagnosis was tearing of the meniscus and the employee is expecting to lose time.
Was a rainy day, slipped on stairs going down to his work area. Wasn't holding safety rails as he said he grabbed the railing to stop himself from falling down the stairs and hit and sprained his knee. He is having an MRI to see if there is further damage on Wednesday 10/24/2012
The incident happened on Oct. 13, 12 @ 1:20 am. EE reported on 10/16/12 that he bumped his head on a pipe in the automated line while packing buckets of zeolite. He never reported to other workers that he was hurt or that he needed to see a doctor. I was not aware of medical treatment of any type until 10/24/12. He was wearing his hardhat at the time of incident.
Employee stepped off B Track Indexer. The water hose he was using to wash up was laying on the ground and he stepped on the hose. This caused his foot to roll to the inside and twisted his ankle. (Sprained ankle)
Driving D41 mantrip at 30 block East Mains when a small piece of rock dust entered right eye causing irritation. Treated with antibiotic/steriod drops to eye. No issue with PPE, training, mining methods or compliance.
Employee was repairing equipment when the rub rail fell off the miner landing on his foot sustaining a strain/sprain and contusion. Employee immediately came out and went to Good Samaritan where they ordered him to remain off work for an uncertain period of time.
The employee was shoveling when he felt a pull in his groin, he was diagnosed with a hernia and required surgery.
When door was removed, the air cannons fired. The force blew hot dust toward ee. EE saw dust cloud and turned to his right exposing the left side of his face. Dust entered between his face shield and his nomex covered face. Some dust entered his left eye.
He compromised the guarding in order to knock dust off the conveyor frame. The hammer he was using was grabbed by the conveyor belt pulling it and right hand between the troughing roller and underside of the belt. Hammer got wedged between the skirtboard and roller causing belt to stall. His right hand, thumb and first two fingers were pinched between the belt and troughing roll.
Was cleaning packed clay and rock from grizzley on screen with a pick. When upper back muscle tighten to a hardness causing pain.
EE was recovering belt structure he took a step forward and felt pain in his right foot.

Employee was operating shuttle car, he hit a hole in the roadway, hitting his head on the shuttle car canopy.
EE was pulling belt head cable when he felt sharp pain in back
Employee was standing under a longwall shield cleaning rocks from the walkway when an emulsion hose ruptured striking him in the right leg, right arm and right side of face (including eye).
Stated: Employee was driving golf cart toward bottom when his steering malfunctioned. His golf cart ran into the rib and he hit his head on the steering wheel and his left leg on the dashboard. He went to the ortho on 10/22/2012 and was diagnosed with hairline fractures to his fibula. He is to remain off work until healed.
EE parked the shuttle car that he was operating, as he was using a pick type hammer to pull come loose material from the rib, his right forearm was caught between the material that he was scaling and the shuttle car. EE continue to work until 10/15/2012 when he was seen by a local physician and placed off work to see a specialist.
EE was getting out of the stockpile truck at the bun and slipped on the platform. The platform became slick since we where pumping gravel that had some clay in it. When EE slipped he hurt his knee.
Employee was checking oil and greasing WA600 loader. Stepped down and missed the step and fell to the ground, hurting lower left side of back, left hip and left leg. Employee thought he would be okay, so he did not go to the doctor at the time of the incident. On 11/27/2012, employee was still in pain and was sent to the doctor for evaluation.
Employee was dismounting his tracked ride into the mine, when he stepped off the ride and tripped over a water line that was lying on the ground and felt a pain in his right ankle.
Switching seats in shuttle car, employee grabbed handle and when he pulled himself up he felt a pain in his shoulder
Employee adjusted the loader hydraulic limits and while stepping between hand rails felt pain in his left shoulder.
Employee fell off of a 4-wheeler. When he fell he fell on his S.C.S.R. and fractured 2 ribs on his left side.
EE stated he fell off ladder while hanging ventilation curtain.
Employee was performing routine maintenance on the propel motor on shovel 4 when he experienced electric shock.
While loading straps onto bolter felt a sharp pain in lower back.
Employee was working on a set of multi media filter that were on a utility trailer. He was standing on the piping that connected the filters together, slipped and fell hurting his right knee, neck and back.
Employee was helping lower a box to the mine floor when the loader bucket hit a crib that was laying on the mine floor and the crib flipped up hitting the employee in the right knee.
While climbing down the ladder from a rock hauler, ee felt something ""pull"" in his left shoulder.
Initial injury 4-10-12. EE sought treatment 4-14-12 & released full duty. 10-16-12 he brought in a doc note to excuse him from work until further notice. he stated he was having continuous pain.
A piece of metal slid against victims right ankle, fracturing his lower leg, just above his ankle. He was treated and released at a local hospital and is scheduled for surgery in the near future and is expected to make a full recovery.
crew was moving track switch timber, everyone threw timber down unexpectedly as employee was still holding on and he pulled a muscle in his back.
EE was carrying 5 gallon cans of oil when he twisted his back.
Employee was prying the cap out of a planetary on a roof bolter, he slipped and fell and twisted his left knee. He continued to work until he could not stand the pain any longer, he scheduled meniscus surgery on 11/20/2012, he has been off work since the surgery.
As miner was attempting to sit in a chair, chair collapsed, causing the miner to fall to the floor.
Employee was walking underground, he stepped on uneven mine floor twisting his left knee. He began missing time on 1/23/2013.
Employee complaining of soreness in right elbow.
Insured worker and another emp were unloading 40ft long pvc pipe when emp lost handle on pipe and it hit him on his right knee. Emp missed 3 days of work. Emp returned to work on 10/13/12.
Employee was bending over to pick up the miner cable and water line, to hang it. As he bent over he felt a extreme pain in his lower back, shooting down his right leg. This employee kept working until he had surgery on 02/25/2013.
EE was performing checks on the dust sampling device being worn by the miner operator. When he walked by the turn table of the machine a rock meas. 74"" x 39"" x 8"" thick fell and struck EE on the left side of his body, resulting in a chipped distal femur, fractured knee, tibia, fibula and multiple lacerations to the face and head.
Employee inadvertently activated the rotation causing the roof bolt to spin and strike his wrist.
A 64 year old Miner with over 26 years experience was doing a hoist resocket on a dragline. As he raised the sledge hammer to knock out the wedge from the socket he felt a popping sensation in his right bicep. The employee visited several doctors getting a second opinion and missed his first day of work on 11-5-2012
The employee was lowering material, loaded into the skip, down the timber slide when the sheave block came free from the rib striking him on the cheek bone and nose.
Employee was riding in transportation bus to work site when a back pack fell from overhead storage and struck her on the head. Medical evaluation and full work release received on 10/09/2012. On 10/15/2012, employee reports symptoms possibly from the October 9th incident and was re-evaluated on 10/16/2012.
Employee was helping change batteries on a scoop when the winch hook slipped catching his hand and spraining his thumb and hand
Employee was helping to install pump on wet plant and used conveyor leg as a ladder, upon coming down slipped and fell about 5 feet to ground, injuring left arm/wrist, and left leg.
Had his hand in box when another co-worker threw a rock in the box and struck the top of his hand.
EE picked up a carry bag containing a chainfall and felt a pain in abdomen. EE checked out by a doctor and cleared to return immediately back to work. The doctor stated that he had a hernia and noted limitations, none of which would prevent him from doing his assigned job tasks. EE came to work on 10/15/12 with a new form from a new dr stating the limitations/no work w/hernia.
While cleaning wall, loose material fell from top, hit left corner of loader bucket, throwing operator forward, causing pain in back of neck
Employee stated that while getting on the drill, when he put his foot on the first step he got dizzy and fell backwards. This caused him to twist his left knee.
Employee was operating a roof bolter installing a channel for a belt header, as he rotated the bolt, the channel caught on the bolt, striking him on his right wrist, causing a fracture. He began missing time on 10/09/2012.
Employee was picking up a brattice block and felt pain in lower back.
Walking down inclined conveyor walkway, tacked grating broke loose causing open hole which employee fell through to ground (Approx 10 feet)
He was installing a cable bolt the bolt caught as it was being pushed into the hole. He spun the bolt to try to free it up. He caught his right wrist on the spinning plate.
The employee was moving a two wheeled cart when he felt a pull in his groin, he was diagnosed with a hernia and required surgery.
EE was attempting to unlatch his tailgate when the tailgate released and the truck moved backing onto the EE's foot, causing injury to his right foot.
While moving a section feeder, the metal side bat wing broke off and landed on employees leg and ankle causing a fracture to his lower leg.
Employee was working on the 17-2 battery scoop in the 1st West Raises section. As he attempted to place the cover on top of the machine into place, when he lifted the cover and twisted to slide it into place, he felt a sharp pain in his left shoulder.

Draw rock falling in 12' cut rock was scaled down bolter was pulled into cut to put corner bolt in more rock fell on canopy slid off striking him in Lt lower leg.
Employee was towing a skid of belt from the surface to the section when the chain holding the belt came unhooked and the belt slid off of the skid. The employee then tried to lift the belt back onto the skid by himself, resulting in a hernia.
Employee used plasma cutter to notch out some tubing. Employee did not use adequate lens for task, resulting in flash burn to his eyes. Employee did not realize he had received flash burn until later that evening. Employee missed one shift (10/7/12) as a result.
Employee was removing a bent roof bolt out of mine roof. The employee reached back to drop drill head and moved the joystick for drill rotation. Bent roof bolt spun striking employee in left wrist. PPE was not a factor, compliance rules/regs was a factor, mining equipment/system was a factor, miner concentration was a factor.

Employee reached down into his box to retrieve a small toolbox and when he pulled up on the small toolbox felt a burning sensation in his genital area. Employee went to CAMC Memorial on 10/06/2012 and was diagnosed with a direct inguinal hernia and orchitis.
While employee was changing bits in the cutter head, the hammer he was using hit the chisel and slipped off and struck the back of his hand. He was holding the chisel in his left hand, the hammer was in his right.

Instead, the operator moved the forklift back as the supervisor was removing a piece of rebar. The rebar dislodged a piece of plate which fell onto the assisting employee's foot.

He received an MRI on 1/10/2013 and surgery for a herniated disc was performed on 2/14/2013.

Employee parked this bus into S-3 switch and walked back to switch to throw straight. As he was walking back to the bus he tripped on a donut crib and fell on his right shoulder. Started losing time on 10/3/2012.

See Attached Was assisting other EE's in grinding on end of a spool. Co-EE heard a popping noise and the end of the pipe he was working on shifted. When he looked to the IEE he saw EE's head pinned to the concrete by the end of the spool (approx 2,700 lbs.) Co-EE's used forklift to hoist the spool off of the EE. 911 called rescue team transported EE to hospital.

Third party worker was transferring bags of barite material to a new pallet because there was a busted bag at the bottom of a stacked pallet. Worker felt discomfort in his right shoulder blade. Worker was sent to medical by his agencies company services.

During his descent his left foot impacted a walkboard fracturing his left ankle.
Employee was operating Euclid R40 and had to do some maintenance. He climbed down ladder, then as he was climbing back up ladder, he got to the top rung and his foot slipped. He grabbed himself with the grab bar with his right hand.

Employee was driving a pin out of a conveyor when he struck his left thumb with the hammer. Injury was a fracture of the left thumb.
The injured employee was struck by a wooden post that had come under compression from the scissor deck he was operating. The post was left leaning against the rail of the scissor deck. When the deck was raised, the post became wedged between the deck and the rib, the pressure was relieved suddenly from the base of the post causing it to strike him in the elbow and ribs.
Helper was changing out a ceramic pump liner. In the process of removing the liner, the liner shattered in the Helper's hand and the pieces when through his glove and cut his thumb.
Shutting grill on truck after cleaning radiator and grill came down on left pinky causing a laceration that required 8 stitches.
On October 29, 2012 employee (Sr. Electrician) was changing out a size three starter bucket. He bent over to pick the starter bucket off from the floor of the MCC room and in the process of raising back up felt a pop/pain in his lower back. He was taken to the doctor and diagnosed with a lower lumbar strain, he returned to work the same day on light duty.
Employee was walking down stairway and his foot slipped off the edge of the last step onto a landing. This caused him to fall against the handrail, and hit his left shoulder on the handrail. Employee was put on restrictive duty.

EE was standing away from the door but received minor burns.
EE was sitting on a bucket and welding overhead. Several pieces of welding slag fell and landed in his groin area. EE sustained 2nd & 3rd degree burns in his groin area. EE placed on restricted duty.
Injured employee was standing on top of the gearbox pedestal and was using a pry bar to separate the coupling halves. While he was applying pressure to the coupling, the pry bar came loose and employee fell backward approximately 3'. When he landed he struck his head against some spare idlers that were staged for a separate job. This resulted in a 3' gash to the top of his head.
EE was climbing ladder and slipped catching himself with arm and straining shoulder.

4 employees were staging 54"" return rollers for replacement on the #7 Lang Main Line belt. One employee was pushing the roller to the side of the tractor bucket, then another employee would lift the roller and slid the roller out the side of the bucket. The roller inadvertently shifted in the bucket and the employee's finger was smashed between the bucket and the roller.

In doing so the employee placed his left hand on the wall to climb up, mean while the employee on the outside engaged the impact gun to loosen the nut. The counter sunk bolt spun and lacerated the left palm.
Ee was removing pin on 972G loader. The bucket and link were removed. Ee was pulling pin out of the top z-bar link where it attaches to the tilt cylinder. Ee pulled pin out and the z-bar shifted because pin was slightly over center. The Ee's thumb was caught between the head of the pin and the bucket leveler bracket.
While lifting a starter from engine block the employee felt a stabbing pain or pop in his right elbow.

The man was walking near the load center when he slipped on the wet bottom and twisted his knee.
Applying wheel chock to truck trailer, felt pain in his right elbow. Employee lifts the wheel chocks 3-4 times per day on average while conducting CDL driver job duties. Employee did not report incident until 10/22/12 to employer and did not seek medical attention until 10/24/12 at which time this became a reportable injury due to medical treatment.
Employee was guzling rock next to a conveyor and when he pulled on hose his elbow hit metal railing. Bumped right elbow employee did not go to doc until 11/6/2012
Employee was operating a forklift near a bulk Dust Foam tank when he ran over a PVC valve on the tank, breaking it. Some of the tank's contents began to leak which he attempted stop by using a rag to plug the valve's broken pipe. As he reached down to plug the leak, he cut his finger on a jagged piece of broken PVC pipe.
Employee loss balance when climbing down out of a front end loader. The employee landed on left shoulder and scraped left knee. The employee thought his shoulder was o.k, he went to his doctor on 11/15/12 and the doctor gave restrictions of *avoid lifting overhead and *avoid lifting over 25lbs until 12/5/12.
EE was drilling when drill became hung up on metal bracing & torque of drill twisted left arm. On 10/31/12 EE asked for medical treatment by MD.
Employee was turning water valves inside of dozer and caught his right thumb on a pulley causing a laceraion which required stitches.
Employee was cleaning belt from machinery and suddenly felt an insect bite on right arm.
The employee was walking down a small grade. The ground was wet and there was loose rock. He slipped on the loose rock and fell backwards on to his back. The top of his right wrist struck against the ground. He felt pain a short time later.
While shoveling spillage from under the tunnel conveyor on to the conveyor, the employee felt pain in the left side of his neck. He went to the doctor on 10-17-12, was diagnosed with a neck strain and placed on light duty. He was returned to full duty by same doctor on 10-19-12.
Employee was climbing into 777D truck, when he slipped, grabbed onto the handrails and strained his left shoulder.
The employee was moving miner cable. A rock fell between the coal rib and roof panel hitting him in the head. Stitches were required for the laceration
EE was traveling along road way to another plant. As he was approaching a hill he moved to right side of roadway. As he proceeded the check engine lights came on. He diverted his attention to the lights and front tire came in contact w/ ditch. Causing truck to tip.
Team Member was climbing ladder to access head pulley drive area of conveyor. When he reached for ladder's railing, he jammed his right little finger into ladder's rung support iron. His finger was fractured.
Worker was driving the pin for a nuclear gauge with a small sledgehammer, and during the course of the swing, the sledgehammer struck the edge and deflected, causing a shoulder strain.
Employee was using a 20 pound sledge hammer to close a slide gate on the #3 finish mill feed belt and strained his back. Employee was placed on restricted duty and returned to work.
Employee was cutting a bearing off a shaft, which was laying on the gravel. The employee cutting stepped to his right. The cutting torch passed by the backside of the fire watch employee's left leg resulting in a second degree burn, approximately 3""x5"" with quarter size blister.
Employee was riding a 3 wheeled buggy. He was making a pre-shift check and the buggy ran into a small hole in the roadway causing a jolt that caused bruising to his hip and lower back as he landed on his mine rescuer that was on his belt.
EE was walking out from a process area when he made contact with an overhead pipe. The following day he had soreness on the back of his neck and lower back. On 10/15/12, EE was sent to an offsite physician who placed the EE on restricted duty.
EE was descending the stairs exiting 102 gantry boom. He missed the second step with his right foot putting all of his weight on his left leg. EE stated he felt a pop behind his left knee and a sharp pain. He was transported to medical dept. Knee strain, restricted work duties.
Employee had just finished drilling a hole in the face and as he pulled the steel out of the hole he stepped on a rock and lost his balance. This caused him to lose control of the machine which pivoted forward on the leg and caught his hand in a pinch point. This resulted in a fractured bone in the employee's hand.
Employee was using a pry bar when it slipped, causing him to fall backwards and strike his elbow on the concrete.
While helping guide a loader moving a 14 inch poly pipe the chain slipped over the loader bucket causing the pipe to fall and swept the EE feet and legs out from under him fracturing his left leg
Employee reached with his left arm to help position the cathode strongback and felt pain in left shoulder.
The employee was checking the oil on a CAT 980K and while running a rag down oil dip stick was impaled by a cable spur into his left index finger. He came immediately and reported the incident to Safety Coordinator. After looking at his finger they transported him via ambulance to RMCH in Gallup.
While attempting to secure a nut on a bolt on the bottom of the BB float cell, the employee fell from ladder when he lost his balance. The employee was standing on the second step of a 4 foot step ladder on the ground floor on the East side of the Float Plant. He fell to the ground, striking his left side (rib area), on the water hose of the nearby water cannon.
The Employee was operating a Roof Bolter, stretched reaching backwards and inadvertently stuck his fingers into the A/C vent, contacting the squirrel cage fan.
EE was cutting a boot off of a roof bolter cable and cut his left thumb.
Employee was placing 80 lb stone on pallet; stone slid off pallet and struck employee in left knee. Restricted duty was assigned. **Employee did not seek treatment until 10/11/2012**
EE was removing a crate from a pallet & felt a slight strain in the shoulder. EE returned home and woke up the next morning with a great pain in shoulder.
EE was striking a slide gate with a hammer and he struck the tip of his left pinky finger with the hammer. EE reported the injury as a first aid on 10Oct12 and sought medical evaluation on 11Oct12. EE was diagnosed with a subdural hematoma and fracture to the tip of his left pinky and released to work with restrictions.
Employee was attempting to lift a metal plate and strained his back. He was placed on restricted duty.
Truck driver was injured when hydraulic cylinder failed and bed dropped on truck frame while unloading. Driver was taken to clinic and released with no restrictions. 8 days later driver reported headache and neck stiffness. 2nd visit to clinic resulted in duty restrictions.
Repositioning steel beam to put a sling around it when she felt pain in her right wrist. She did not seek medical treatment until 10/11/12.
The employee was cleaning a Cardox port on the kiln, when the kiln went positive and hot dust hit the employee in the face. The employee sustained burns to the left side of his face and left ear.
Employee assigned to the powder crew was in the process of preparing to load a round when his partner inadvertently lowered a stabilizer jack onto the employee's left foot, resulting in a hairline fracture of the second metatarsal.
Employee stated he strained his back while shoveling mill scale along W3801 belt.
Contractor employee was using a shovel to clean up ore from the fine ore bins when she received a strain to her lower back. Employee received restricted duty.
Employee was removing doors from FFM2 for inspection of interior of mill. When employee lifted the door cross bar, he felt a sharp pain in his back. He was lifting the bar from a kneeling position.
Employee attempting to straighten a steal pin cut the palm of his hand. The end result was two stitches on left palm. The employees stitches will be removed in ten days and until than he will be working on little duty.
Employee exited catwalk and climbed down ladder. He hit his right knee on a hook that used to hold a hanging ladder. The hanging ladder had been removed from service but the hook wasn't. The hook fell between the rungs of the ladder the employee was using. The hook had a sharp edge that lacerated employees knee.
EE was pushing a materials cart and cart struck a part of the conveyor belt. The EE's foot was under the cart and the stress of striking the belt along with the right leg being stuck caused a break in the lower area of his right tibia.
Employee was kneeling on the bank while connecting two 8"" suction hoses. Employee was pulling the two hoses together so they could fasten the coupling clamp and felt a sharp pain on the lower left side of his back.
Employee was replacing the landing gear on a Tanker. While he was carrying the new landing gear over to where the Tanker was located, he felt pain in his back.
Miner was standing on an extension ladder his feet were on 5th rung approximately 5 to 6' off the ground. He was welding just above the ladder on the back of the kress truck. The ladder slipped out from under him. He sprained his Rt. wrist trying to catch him self. Miner stated he failed to tie off the ladder or use the bed platform. Miner was put on restricted duty.
EE was rolling slurry blind on #5-9 heater, when the impact gun that he was using slipped off the nut. The gun pinched employee's right hand ring finger between the gun and slurry blind. EE received a deep bruise to the tip of his finger. X-ray revealed fracture on finger tip. EE placed on restricted duty on 10/08/12
EE was on top of Dust Collector with harness and lanyard; he was getting into position for stack testing when the ducting weld gave way and the duct fell out of position. He put his hand out to steady himself and maintain balance. He jammed his thumb on the ducting, fracturing his thumb.
While stepping out of the South E-crane switch house, EE stepped on a rock and rolled the right foot. EE felt a slight pain in the right ankle and was sent to medical for evaluation. EE placed on restricted duty on 10/08/12.
Employee was dismounting from the back of the water truck and stepped on a small rock in the road and rolled his right ankle causing swelling and a bad sprain.
Employee was getting off the miner and strain his knee.
