A ROOF FALL MEASURING 20' BY 19' BY 6' IN THE NORTH AT X-CUT #1 IN #4 ENTRY. THE AREA HAD BEEN SUPPORTED WITH 6' TENSION REBAR ROOF BOLTS AND WAS CAUSED BY BY A HIDDEN SLIP.

A ROOF FALL OCCURRED IN THE 2 AND #3 ENTRIES APP. 1400' OUTBY 004 SECTION. FALL WAS LOCATED 60' INBY SPADS 6816 & 6817. FALOL WAW 20' X 60' X 4-6' THICK. Y
THERE WAS A VERY LARGE MUD SLIP ACROSS THE INTERSECTION. IT WAS BOLTED WITH 6' FULLY GROUTED RESIN AND HAD 9' COMBINATION BOLTS SPOTTED BETWEEN THE T-5 CHANNELS. THE AREA HAD A ROW OF CRIBS NA RROWING WIDTHS AND DOWN TO 14'. NOTHING WAS NOTICED ON THE WEEKLY EXAMINATION 1 WEEK PRIOR.
THIS WAS AN OUTBY INTAKE AREA, WHERE OTHER FALLS HAVE OCCURRED. THE AREA WAS BOLTED WITH 4' RESIN BOLTS AND THE FALLS ALL ARE 4' HIGH. IF 6' BOLTS WERE USED, THE FALLS--IN ALL LIKELIHOOD--WO ULDN'T HAVE HAPPENED.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 1ST EAST SUBMAIN 4097 SE #6 ENTRY. THE FALL WAS 20' LONG X 19' WIDE X 8-10' THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF MINERS. THE FALL WILL NOT BE CLEANED UP.

WE WERE ON PILLARS SOMETIME ON THE MORNING OF 8/29/00. THE TOP CRACKED LETTING WATER RUN THROUGHTHE ROOF AND WATER FILLED A DIP ON SECTION AND SUBMERGED THE 2480 CONTINUOUS MINER.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON A-3MAINS, NO. 1 ENTRY JUST OUTBY SURVEY STATION NO.367. THE FALL WAS APPROX. 49' LONG X 17' WIDE X 7' HIGH. THE FALL CONTAINED 6' RESIN BOLTS AND THE AREA WAS DEVELOPED 5/98. THE APPROCHES ARE DANGERED OFF AND TIMBERS INSTALLED. FALL WON'T BE CLEANED UP AT THIS TIME.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE GALATIA NORTH MAIN WEST #9 AT 9000 WEST. THE FALL WAS 10'' LONG X 18-19' WIDE X 10-12' THICK. THE FALL DID NOT AFFECT VENTILATION OR PASSAGE OF MINER S. THE FALL WILL NOT BE CLEANED UP.
A ROOF FALL WAS FOUND WHILE WALKING THE WEEKLY EXAM. IT WAS IN #1 RETURN ENTRY, X-CUT 83, 1 WEST SUBMAIN. SIZE WAS APPROX. 50'L, 18'W, 5'THICK. IT WAS CAUSED BY A SLIP. THIS AREA WAS LAST MINED IN 1999.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE#8 ENTRY AT SPAD #3574 ABOUT 3000' FROM THE FACEMEASURING 20'LONG X 20'WIDE X 5'HIGH.
AN AREA OF APPROX 4'-5'X18'WX80'L FELL 10,000 FEET OUTBY THE ACTIVE SECTION ON THE 8 LEFT PANEL. THE FALL WAS LOCATED IN THE #7 MAIN THRU THE CROSSCUT TO THE #8 MAIN (CROSS CUT #83). 4' FULL Y GROUTED BOLTS HAD BEEN USED. IT APPEARS THAT AN AREA OF SLICKENSIDE ROOF WAS THE CAUSE OF THE FALL.
HOIST LOST POWER AT 3:10 PM AND ELECTRICIANS WERE CALLED TO REPAIR IT. MSHA WAS CALLED AT 3:23PM AND TOLD THAT #31 HOIST AT FLETCHER WAS OUT OF SERVICE. THE ELECTRICIANS FOUND ALL AUXILLARY C ONTACT THAT WAS BAD ON THE MAIN LOWER CONTACTS AND CAHNGED IT OUT. HOIST WAS RETURNED TO SERVICE AT 5:29 PM SAME DAY AND MSHA WAS CALLED AND TOLD HOIST WAS BACK IN SERVICE.

ROOF WAS SANDSTONE AND HAD BEEN PREVIOUSLY SUPPOR TED WITH 5' INSTALLED BOLTS CINT ANCHOR. AREA WAS BROKED OFF AND WILL NOT BE CLEARNED UP. NO PERSONS, EUQIPMENT OR VENTILATION EFFECTED.
A FALL OCCURRED IN THE #3 ENTRY ON THE MAIN LINE. THE ROOF FALL IS APPROX 70' LONG 18' WIDE AND 3-5' THICK. THERE WERE NO INJURIES.

CONTRACTOR WAS AIR ARCING ON GEAR CASE AND SPARKS IGNITED GREASE AROUND GEAR AREAS. FIRE BURNED 60 MINUTES DUE TO DIFFICULTY GETTING WATER DOWN IN TO THE CASING. NO INJURIES. DAMAGE WAS MIN IMAL.

HOISTMAN HAD BEEN TAKING SUPPLIES UNDERGROUND. HE BROUGHT CAGE TO SURFACE LANDING AND SPOTTED IT. AT THAT POINT HE HEARD A POPPING NOISE IN MOTOR CONTROL. HE CALLED ELECTRICIANS AND THEY CH ECK IT OUT AND FOUND HOIST MOTOR BD. CALLED MSHA AT 1400.

A ROOF FALL WAS FOUND IN OLD WORKS NEXT TO TRAVELWAY WHILE DOING AN EXAM. IT FELL IN ENTRY 5 AT XCUT 36 IN THE 4R1W UNIT. THE SIZE WAS 40' LONG, 18' WIDE, 7' THICK. IT WAS CAUSED BY A SLIP ON ONE SIDE OF THE FALL. IT DID NOT AFFECT PEOPLE, EQUIPMENT, OR VENTILATION.
WHILE WORKING ON MAIN FRAME INSERTS ON #2 SHORTHEAD CRUSHER, EMPLOYEES CUT A PAD EYE WITH A WELDER; THE SLAG DROPPED ON A TERTIARY SCREEN PANEL, CAUSING A FIRE ON THE TERTIARY SCREEN. STARTED TO DO SHAFT CHANGE OUT JOB; TIED OFF CAGE, RAN SLACK OUT, PUT CONTROL IN NEUTRAL AND LOCKED OUT CAGE; STARTED UP STAIRS AND SAW ROPE MOVING. AFTER ROPE QUIT MOVING, DAMAGE HAD OCCURRE D TO THE CAPPEL, WHICH REQUIRED US TO POSTPONE THE FIRST JOB AND REPAIR THE CAPPEL.

HE THEN WENT IN FRONT OF THE DRILL AND STEPPED OVER THE DRILL AUGER. HE BACAME ENTANGLED WITH SOME PART OF HIS CLOTHING HE GOT PULLED INTO THE DRILL AND BECAME SO ENTANGLED THAT HE COULD NOT BREATH. THIS ACCIDENT IS STILL UNDER INVESTIGATION

THE EMPLOYEE WAS WORKING FROM THE GALLOWAY WORK PLATFORM IN THE BOTTOM OF THE RODEO VENTILATION SHAFT. THEY HAD JUST FINISHED LOADING AND WIRING IN THE BLAST FOR THE END OF SHIFT. THE ORE BU CKET HAD BEEN CALLED FOR SO THAT THE CREW COULD RIDE BACK OUT TO THE COLLAR FOR THE BLAST TIME AND SHIFT END. AS THE ORE BUCKET ENTERED THE BUCKET WELL OF THE GALLOWAY, THE EMPLOYEE LEANED OV
THE WORKER WAS WELDING OVER THE WASHER STANDING ON A SCAFFOLDING. THE WORKER EITHER WAS PULLED OR FELL INTO THE WASHER TO HIS DEATH
THE FATALLY INJ PERSON WAS TRAMMING THE JOY CONTMINER ACROSS THE SECTION WHEN HE STOPPED TO THROW THE MINER CABLE OUT OF THE WAY-HE WAS BETWEEN THE MINER & COAL RIB. IN THE PROCESS OF THROWING THE CABLE HE INADVERNTLY HIT THE TRAM LEVER CAUSING THE MINER TO SLIDE INTO THE RT RIB CRUSHING HIM
EE WAS FATALLY INJURED WHEN HE WAS STRUCK BY A SHUTTLE CAR & CAUGHT BETWEEN THE SHUTTLE CAR & THE COAL RIB. EE AND ANOTHER EE WERE IN THE PROCESS OF ANCHORING THE NO.1 SHUTTLE CAR CABLE ON THE 1ST RIGHT SECTION.
THE DECEASED WAS ASKED TO EXAMINE A CORNER OF A PILLAR BY THE SCALERS. HE GOT IN HI-RANGER BASKET W/O FALL LPROTECTION OVER THE OBJECTION OF SCALER (1 1/2 YRS EXP). WHILE INSPECTING THE PILL AR, THE BASKET BECAME HUNG UP ON THE SIDE OF THE PILLAR, WHEN IT BROKE FREE. THE BOOM SPRUNG AND THREW BOTH MEN OUT. THE SCALER WAS SAVED BY HIS FALL PROTECTION. THE POWDERMAN FELL APPROX 5
MAN WAS DRIVING 922B LOADER DOWN A ROAD WITH A STEEP GRADE. FOR UNKNOWN REASON, LOST CONTROL AND THE LOADER TURNED OVER AND THE RAPS SAFETY CAB FAILED AND COLLAPSED CAUSING FATALITY.
THE VICTIM, EE, AGE 7 WAS COLLECTING QUARTZ CRYSTALS AT THE BASE OF THE HIGH WALL WHEN THE OVER BURDEN COLLAPSED STRIKING THE VICTIM KNOCKING HIM TO THE GROUND.
EMPLOYEE WAS SLEEPIGN ON A MUCK PILE WITH HIS LIGHT OFF. LHD 9OPERATOR WAS UNABLE TO SEE HIM AND LOWERED THE BUCKET ON HIM.
THE INJURED WAS OPERATING THE CONT. MNER WITH A REMOTE BOX AND SITTING UNDER A KETTLE BOTTOM. THE KETTLE BOTTOM FELL STRIKING HIM IN THE HAND & FINGER.
EMPLOYEE SAID WHEN HE WENT TO GET HIS CDL PHYSICAL, THE DOCTOR TOLD HIM HE HAD A HERNIA. IT HAS INCREASED IN SIZE. HE DID NOT KNOW WHEN THIS HAPPENED. HE WENT TO DOCTOR 6/28/00. WAS REPORT ED TO ME 8/28/00.
EMPLOYEE GOT HIS FINGER CAUGHT BETWEEN METAL CABLE AND SHIEVE WHEEL ON BELT TAKE UP. CAUSE OF INJURY INVOLVED AWARENESS AND MINER PROFICIENCY AND TWO SKILLS. ACCIDENT DIDN'T INVOLVE PROTECTI VE DEVICES OR PROTECTIVE CUTTING; IT WAS NOT INVOLVING ANY DEFECTIVE EQUIPMENT OR MINING SYSTEM.
EMPLOYEE STEPPED UP ONTO THE FOURTH PALLET HIGH IN A STACK OF PALLETS SO I COLUD REACH A LOOSE BOARD AT THE TOP OF THE STACK. AS EMPLOYEE GRABBED THE BOARD, THE RING ON LEFT RING FINGER GOT CA UGHT ON A NAIL AND AS EMPLOYEE STEPPED OFF THE PALLETS LEFT RING FINGER WAS AMPUTATED.
THE SECTION CREW WAS ADVANCING THE SECTION POWERBOX AND CABLES. THE EMPLOYEE WAS PULLING 7200V HIGH VOLTAGE CABLE UP TO BOX. HE STATED HE FELT A SHARP BURNING PAIN IN THE GROIN AREA.
EMPLOYEE HAD REMOVED THE CAP ON THE ROTARY AIRLOCK FEEDER TO INSPECT IT. HE WAS USING HIS FINGER TO CLEAN OUT THE OPEN SECTION. HIS FINGER WAS CAUGHT BETWEEN THE FEEDER AND THE FEEDER HOUSING, PINCHING FINGER OFF AT FIRST KNUCKLE.
PIECE OF METAL FLEW OFF MINER CONVEYOR CHAIN WHILE 2 REPAIRMEN WERE WORKING ON IT AND INJURED WAS WALKING TOWARDS MINER WHEN A PIECE OF METAL HIT HIM IN THE EYE.
INJURED PICKED UP A 6"" WATER LINE AND RAISED IT ABOVE HIS HEAD TO HANG ON CHAINS. HE FELT PAIN IN HIS STOMACHE AND BACK.
EMPLOYEE WAS LIFTING A CONCRETE FORM (8' BY 2') BY HIMSELF WHEN HE FELT A PAIN IN HIS STOMACH.
WHILE REMOVING MATERIAL FROM FLOW GATE, WITH LEFT HAND, HE REMOVED MATERIAL THAT WAS HOLDING GATE UP. THE GATE CLOSED ONTO LEFT THUMB; PARTIAL AMPUTATION.

EMPLOYEE WAS USING A PIPE TO BEAT ON A FEEDER AND FEED CHUTE. THE PIPE SLIPPED OUT OF HIS HAND AND HIT FINGER. THE FINGER WAS PINCHED BETWEEN THE FALLING PIPE AND A PIECE OF TIN.
WHILE PERFORMING NORMAL WORK DUTIES EMPLOYEE EXPERIENCED PAIN AND BURNING IN THE GROIN AREA. HE WAS TRANSPORTED TO HOSPITAL AND DIAGNOSED AS HAVING A HERNIA.
EMPLOYEE WAS ASSISTING IN REPLACING BELTS ON A CRUSHER. MECHANIC INSTRUCTED WORKERS TO PULL HANDS BACK BEFORE ROLLING BELTS ON. EMPLOYEE KEPT HIS HAND NEAR THE BELT, AND IT WAS PINCHED BETWEEN THE BELT AND PULLEY. THUMB WAS CRUSHED AND APPRROX 1/4 INCH HAD TO AMPUTATED.
EMPLOYEE STATED HE HAD ATTEMPTED TO LIFT AND MOVE A PORTION OF THE CONVEYER BELTING. THIS IS VERY HEAVY MATERIAL AND SHOULD ONLY BE MOVED WITH A FORKLIFT. HE STRAINED THE GROIN AREA AND IS NOW LOOKING AT SURGERY TO CORRECT THE PROBLEM. HE WILL BE OFF FOR APPROX 4-6 WEEKS.

EMPLOYEE WAS STANDING ON FLATBED TRAILER HELPINGTO UNLOAD SMALL TIRES. THE EMPLOYEE TOOK A STEP BACK AND TRIPPED OVER A TIRE. THIS ACTION CAUSEDHIM TO FALL AND HIT HIS HEAD ON THE TRAILER AND

A SPIRAL MANIPULATION WAS ADMINISTERED BY DOCTOR TO RELIEVE PAIN; EMPLOYEE WAS ADVISED TO STAY HOME FROM WORK AT LEAST ONE DAY TO AVOID RE-AGGRAVATION OF INJURY.

EMPLOYEE STATED THAT HE HAD FINISHED HIS WORK SHIFT AND WAS EXITING HIS HAULER. HE STATED THAT HE HAD NOT HIT OR TWISTED HIS KNEE IN THE PROCESS. HOWEVER, AS HE WAS CLIMBING DOWN THE HAULER LA DDER HE FELT A BURNING SENSATION IN HIS RIGHT KNEE. AFTER REACHING THE GROUND, HE STATED THAT THE BURNING EASED OFF. ON 8-23 HE MISSED WORK DUE TO KNEE AND WENT TO DOCTOR.
WHILE CLIMBING DOWN #935 MAC DUMPTRUCK, EE'S LEFT FOOT SLIPPED OFF OF THE TIRE CAUSING A SPRAIN. EE DID NOT FALL AND IS NOT SURE IF HIS FOOT WAS INJURED ON THE TIRE OR WHEN HE STEPPED ON THE P AVEMENT. HE WAS PUT ON RESTRICTIONS FOR APPROX. 5 DAYS.
EE WAS SHOVELING MATERIAL FROM UNDER CONVEYOR BELT ONTO CONVEYOR BELT. HIS SHOVEL HT A ROLLER. THE SHOVEL KNICKED BACK AND HIT EE ON HIS RIGHT CHEEK. THE BLOW CUT HIS CHEEK. EE RECEIVED 19 STITCHES.
MECHANIC WAS CHANGING TRANSMISSION SCREEN LOCATED UNDER PUMP IN D11 TRACTOR. HAD TO GO THRU FLOOR BOARD OPENING TO GET TO THE TRANSMISSION SCREEN, WHILE CLIMBING DOWN THRU THE OPENING, LOST FO OTING ON OILY HOSE, AND FELL, WHILE TRYING TO CATCH HIMSELF CAUSED HIM TO STRAIN HIS LOWER BACK MUSCLES.

IT SHOULD BE NOTED THAT INJURED IS ABOVE AVERAGE IN SIZE AND MAY HAVE HAD TROUBLE IN SMALL OPERATOR COMPARTMENT.

EE WAS WORKING ON THE SAND SCREW ALL DAY & BUMPED HIS RIGHT LEG AGAINST SOMETHING & BY THE END OF THE DAY IT WAS RED & BRUISED & SWOLLEN.
GETTING DOWN FROM LOADER, HE STEPPED FROM THE BOTTOM STEP ONTO THE GROUND AND HURT HIS RIGHT KNEE. DISLOCATED IT? WENT TO THE DOCTOR AND WAS PUT IN A BRACE.
THE EMPLOYEE WAS LOADING A 3 TON CAPACITY CHAIN RATCHET INTO A SCOOP BUCKET. WHEN HE SWUNG THE RATCHET INTO THE SCOOP, HE FELT A SHARP PAIN IN HIS RIGHT KNEE.

EMPLOYEE WAS HELPING WITH A BELT MOVE AND BEGAN EXPERIENCING PAIN IN HIS LOWER BACK. EMPLOYEE HAD LIFTED SEVERAL TIMES DURING THE SHIFT OF WORK AND COULD NOT PINPOINT THE CAUSE OF HIS INJURY.
ROOF BOLTER WAS STUCK AND SCOOP WAS PUSHING ROOF BOLTER OUT AND SCOOP SLIPPED UNDER RIB AND PUSH PIECE OF METAL IN CANOPY AND PINCH LEFT LEG AND CAUSE DEEP BRUISE TO LEFT LEG.
EMPLOYEE WAS USING A LONG STEEL BAR TO DRIVE OUTA LOADER BUCKET PIN.
EE WAS COMING IN TO WORK WHEN A COAL TRUCK TURNED AROUND ON THE HAUL ROAD AND HAD NOT GOT BACK TO HIS SIDE OF THE ROAD. EE'S PICKUP TRUCK HIT THE COAL TRUCK HEAD ON.
HE WAS HELPING MINER OPERATOR SET LIFT TIMBERS IN #5 ENTRY WHEN A KETTLE BOTTOM PINCHED OUT BETWEEN TWO ROOF BOLT PLATES. HE HAD HIS BACK TURNED AWAY FROM IT AND DIDN'T SEE IT. PART OF IT ST RUCK HIS BACK SIDE. HE WAS TAKEN TO HOSPITAL ER AND THEY X-RAYED HIM, CHECKED HIM AND SENT HIM HOME.
INJURED LEFT SHOULDER WHILE THROWING TRACK SWITCH. MAN LOST TIME UNTIL 9-25 WHEN HE HAD SURGERY.
EMPLOYEE WAS PREPARING TO SERVICE THE 980 LOADER TO GET TO THE ENGINE YOU HAVE TO PULL OPEN FENDERS. IN PULLING ON ONE OF THE FENDERS HE HURT HIS BACK. 2 WEEKS LATER HE COULD NOT WORK.
EE HAD BEGUN CLIMBING A LADDER. HE HAD 2 SODAS IN HIS LEFT HAND AND WAS THEREFORE USING HIS LEFT ARM ON THE LADDER RUNG RATHER THAN HIS HAND. HE BEGAN WITH HIS LEFT FOOT ON THE 1ST RUNG AND RT . HAND ON A RUNG ABOUT HEAD HEIGHT. WHEN HE TRANSFERRED HIS WEIGHT ON RT. FOOT ON 2ND RUNG HIS FOOT SLIPPED CAUSING HIM TO FALL ABOUT TWO FEET TO THE GROUND CAUSING A SPRAINED ANKLE & CRACKED

SHE MISTIMED THE TROLLEY AND GOT HER RIGHT LEG CAUGHT BETWEEN THE TROLLEY CAR AND CONVEYER. SHE IS A TEMP EMPLOYEE WITH 2 WEEKS EXPERIENCE IN JOB.
WHILE OPERATING THE #62 SHOVEL, OPERATOR FELT A BUG (INSECT) ENTER HIS LEFT EYE. ON 8-21-00 HE SAW A PHYSICIAN, THE PHYSICIAN PLACED HIM OFF WORK ON 8-22-00, FOR EYE INFLAMATION.
WAS DOING A MINE EXAMINTION (PRE-SHIFT) OF A BELT LINE. AS HE WAS WALKING HE SLIPPED ON THE WET BOTTOM AND BEGAN TO FALL. HE USED HIS LEFT ARM TO CATCH HIMSELF AND SPRAINED/STRAINED HIS LEFT ELBOW.
HEAVY STEEL PLATE FELL ON THE BACK OF EE HURING HIS BACK & HIP. EE THOUGHT THE PAIN WAS DUE TO RHIS REOCCURANCE HERNIA AND DID NOT REPORT THE ACCIDENT TILL AFTER HIS VISIT TO DR WHO DETERMIN ED THE INJURY TO HIS BACK & HIP\
TRK DRIVER WAS STEPPING OFF A TRAILER & FOOT WAS CAUGHT IN THE STEP & STRAINED HER ANKLE
EMPLOYEE STATED HE WAS ON HIS KNEES AND TURNED TO PULL MINER CABLE AND HEARD POP PAIN IN KNEE.
A DRILLER WAS ATTEMPTING TO REMOVE THE DRILL HAMMER WITH A HYDRAULIC CHAIN WRENCH. WHEN PRESSUREWAS APPLIED TO LOOSEN THE HAMMER, THE NUT FLEW OFF AND STRUCK THE DRILLER ON THE EAR & MASTOID BONE. HIS EAR WAS LACERATED AND BRUISED ON THE MASTOID. THE LACERATION WAS SUTURED WITH THREE SUTURES.
EE WAS OPRATING A RETRIEVER PULLING ON A MINER THAT CAUGHT IN A PILLAR FALL WHEN ONE OF THE LINKS BROKE AND A PIECE OF METAL HTI HIM IN THE FACE.
THIS INCIDENT WAS REPORTED ON 8-29-00. CONDITION BEGAN ON 8-19-00 AS EMPLOYEE WAS WORKING ON THE TUBE BELT IN CANLINE. HE EXPERIENCED FATIGUE AND CRAMPS. HE RESTED AND CONTINUED TO WORK. CONDI TION CONTINUED TO DEVELOP OVER SEVERAL DAYS. DOCTOR DETERMINED HEAT STRESS.

EMPLOYEE WAS INSTALLING A RAM JACK PIN ON A SHIELD. THE EMPLOYEE SLIPPED ON A PIECE OF SLATE, LOST HIS BALANCE. THE RAMJACK FELL ON THE EMPLOYEE'S RIGHT INDEX FINGER.
WAS OEPRATING A REMOTE CONTROL MINER. A PIECE OF RIB FELL OUT BETWEEN RIB BOLTS AND STRUCK HIM ON THE BACK PUSHING HIM INTOT HE MINER.
EMPLOYEE WAS WALKING NEAR CO-WORKERS WHO WERE REMOVING A BEARING FROM THE STACKER BELT TAIL ROLLER. AS THE BEARING WAS STRUCK WITH A SLEDGE HAMMER, A PIECE OF BEARING STRUCK EMPLOYEE IN THE LE FT FOREARM.

SUBJECT WAS SHOVELING SECTION TAILPIECE. SHOVEL CAUGHT A SPLICE ON THE BELT, FORCING IT AGAINST THE TOP AND CATCHING HIS LITTLE RING, AND MIDDLE FINGERS OF HIS LEFT HAND, BETWEEN THE SHOVEL AN D THE ROOF.
THE EMPLOYEE WAS HELPING PULL LONGWALL TAILGATE PAN DOOR TO BE LOADED ON FLAT CAR. THE DOOR WAS BEING DRAGGED BY THE SCOOP. A CORNER OF THE DOORHUNG ON A RAIL. THE DOOR SLID AROUND AND SLID O N THE FEET AND LEGS OF THE EMPLOYEE.
WORKER WAS CHANGING A FEED TANK VALVE AT 7FT ABOVE FLOOR LEVEL. WHILE HE WAS HAMMERING HE LOST HIS BALANCE HE CAUGHT HIMSELF WITH HIS LEFT ARM CAUSING A SHOULDER STRAIN

EMPLOYEE WAS HELPING MAKE A BELT SPLICE. HE TRIED TO REACH IN AND REMOVE SPLICE PAPER AND WAS STRUCK BY A HAMMER. FRACTURE TO RIGHT RING FINGER.
AFTER GREASING JAWS I BACKED DOWN LADDER, SLIPPING ON STEP AND FELL ABOUT 4 OR 5 FEET LANDING IN A SITTING POSITION.
INJURED WAS TRAVELING TO SECTION ON A 4 WHEELER MANTRIP. HE RAN OVER A ROCK IN THE ROADWAY AND FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE WAS HOLDING A PNUEMATIC WATER PUMP FROM WAIST HEIGHT 2-3' WHEN HIS FEET SLIPPED APART IN THE MUD AND GRAVEL AND DROPPED THE WATER PUMP ON HIS LEFT FOOT.
