THE FALL OCCURRED DUE TO EXCESSIVE PRESSURE ABOVE THE INSTALLED 5' COMBINATION ROO F BOLTS. ENTRANCES TO THE FALL WAS RE-SUPPORTED WITH ADDITIONAL SUPPORTS (CRIBS). THE FALL WAS RE-SUPPORTED WITH 5' COMBINATION ROOF BOLTS AND 10' ROPE BOLTS.

A ROOF FALL HAS OCCURRED IN THE HEAD GATE ENTRY OF THE C-3 LONGWALL PANEL JUST OUTBY THE LONGWALL FACT AT SURVEY STATION 6948. THE FALL MEASUREDUP TO 86 FT IN LENGTH, 20 FT IN WIDTH, & APPROX 9 FT HIGH. THE FALL MATERIAL CONSISTED OF APPROX3.5 FT OF LIMESTONE & SAND, SHALE 5 FT ABOVE. THE AREA HAD BEEN BOLTED WITH 4' RESIN BOLTS
DURING INSPECTION OF THE ESCAPE HOIST #6 VENT SHAFT. IT WAS FOUND THAT THE PROGRAM LOGIC CONTROLLER WAS DOWN. ALL SHIFTS WERE NOTIFIED THAT THIS ESCAPE SHAFT WAS INOPERATIVE AND PARTS WERE ORD ERED. REPAIRS WERE MADE ON 1-2-01.

A ROCK BURST OCCURED IN THE 6020 RAMP (06 DOWN RAMP) WHICH REGISTERED 2.8 ON RICHTER SCALE. DAMAGE MAINLY TO RIGHT RIB. ABOUT 65' OF RIB, SOME BEYOND ANCHORAGE OF 4' SPLIT SETS. NO ONE WAS IN THE WORK AREA AT THE TIME OF THE BURST AND NO ONE WAS INJURED.

THE FLAME WAS EXTINGUISHED USING ROCK DUST AND A 20 POUND FIRE EXTINGUISHER. NO INJURIES OR PROPERTY DAMAGE OCCURRED.

THE LONGWALL FACE HAD ADVANCED APPROXIMATELY 160FEET WITHOUT A SUBSTANTIAL CAVE. WHEN THE ROOF FRACTURED THE RESULTING CAVE WAS ASSOCIATED WITHAN AIRBLAST THAT DAMAGED UP TO 20 PERMANENT VENT ILATION CONTROLS WHICH HALTED MINING FOR NEARLY 24 HOURS.
TOP HAD DETERIATED OVER A LONG PERIOD OF TIME, SOME DRAW ROCK HAD FELL FINALLY SOME HAD CAME LOOSE FROM THE ANCHORAGE. FALL WAS CLEANED UP AND SUPPORTED BACK. THIS MINE IS BEING REBOLTED IN AL L AREA'S. THERE IS NO PRODUCTION AT THIS MINE.
MINER OPERATOR STATED THAT HE THOUGHT HE SAW A YELLOW FLAME OVER THE RIPPER ON THE 12-12 MINER.HE WAS CUTTING THE TOP WHICH WAS SANDSTONE.

ABOUT 2100 ON 12-13-00 ON THE #2 SECTION, THE MINER WAS CUTTING #3 RT. BREAK WHEN WATER BEGAN TO APPEAR ALONG THE MINER. IT CONTINUED & THE SECTION CREW BEGAN MOVING EQUIPMENT TO HIGHER GROUND . THE EMPLOYEE WAS USING A RAG TO WIPE OIL FROM THE BODY OF A COMPRESSOR. THE SUCTION FROM THE FAN CAUSED THE RAG TO BE DRAWN INTO A SMALL AREA BETWEEN THE GUARD AND THE FAN MOUNTING. THE FAN BL ADE THEN CONTACTED HIS FINGER CAUSING THE INJURYWHICH REQUIRED SUTURES TO CLOSE.

HE IS STILL WORKING BUT NOT LIFTING HEAVY OBJECTS AND IS LOOKING TO REPAIR THE INJURY BY SURGERY. THIS IS DETERMINED TOBE AN OCCUPATIONAL INJURY.
CLIMBING ON FRONT END LOADER TO FIX RADIO ANTENNA FELL OFF WHEN FOOT SLIPPED ON WET SURFACE FROM HEAVY DUE.
EE WAS WALKING TO JOBSITE AND SLIPPED ON LOOSE ROCK AND PULLED LOWER BACK.
WHILE CHANGING JACK ON MRS, HE STEPPED ON LUMP OF COAL TWISTED LEFT KNEE.
WALL FELL ON LEFT SIDE OF CAB BECAUSE OF WET CONDITIONS. EE WAS TAKEN TO HOSPITAL AT REGIONAL MEDICAL CENTER RELEASED WITH BRUISED.
EMPLOYEE WAS ATTEMPTING TO LIFT OR MOVE #4 CONVEYOR BELT. WHILE ATTEMPTING TO LIFT, HE FELT A PAIN IN THE LOWER GROIN AREA. WHILE HE FELT PAIN, HE DID NOT CONFIRM A HERNIA UNTIL 1/30/01 THROUG H A FAMILY DOCTOR. THE ARK. WORKERS COMP. DENIED THE CLAIM. HOWEVER, ASH GROVE AGREED TO COVER UNDER WORKMANS' COMP. AND THE SURGERY WAS DONE ON 5/29/01. THAT WAS HIS FIRST DAY OF LOST TIME.
HAD COMEALONG HANGING UP WAS PULLING SLACK OUT OF CHAIN WHEN PLASTIC TAB ON END OF CHAIN CAME UP & HIT HIM IN EYE
INJURED WAS STRUCK ON THE LOWER LEG BY A HYDRAULIC UNIT THAT HE WAS CARRYING.
SHUTTLE CAR CANOPY TWISTED/RETATED DOWN FROM IT'S POJECTION AND PINCHED HIS HEAD AND NECK AGAINST THE DECK OF THE SHUTTLE CAR. 2 BOLTS HAD BEEN REMOVED TO ALLOW CANOPY TO BE ROTATED DURING M OVING OF EQUIPMENT FROM ONE SECTION TO ANOTHER.
WHILE EXITING SCOOP EMPLOYEE TWISTED HIS RIGHT KNEE.
EMPLOYEE WAS SETTING A TIMBER WHEN A PIECE OF ROCK FELL BETWEEN ROOF BOLTS, STRIKING EMPLOYEE ON LEFT LEG.
GETTING PARTS OUT OF STORAGE VAN. SLIPPED AND FELL OUT VAN(4 FT.) LANDED ON LEFT SHOULDER/ELBOW.
EE CLAIMS THAT WHILE BEING LOADED BY THE P & H 4100 SHOVEL THAT A PARTICLE OF DUST CAME THROUGH THE OPENDED WINDOW OF THE HAUL TRUCK AND LANDED IN HIS RIGHT EYE.
THE ENGINE SUPPORT CROSS MEMBER WA ON THE FLOOR JACK IN FRONT OF THE NEW ENGINE. EE WAS WALKING AROUND FRONT OF ENGINE TO THE LEFT SIDE. HIS RIGHT SHIN BUMPED THE CROSS MEMBER CAUSING IT TO START TO FALL. HE PUT PRESSURE AGAINST THE MEMBER TO KEEP IT FROM FALLING OVER. THE CROSS MEMBER SUPPORT TIPPED OVER SCRAPPING HIS SHIN AND LANDIN GON HIS FOOT BEHIND THE STEEL TOE.
EMPLOYEE WAS STRUCK ON HIS RIGHT MIDDLE FINGER BY A FALLING VENTILATION TUBE.
EE WAS INJ 12/00 NO LOST TIME. L SHOULDER HAS GOTTEN MORE PAINFUL & EE WAS SEEN BY AN ORTHOPEDICSURGEION WHO RECOMMENDED SURGERY. EE HAD SURGERYON L SHOJLDER 5/29/01 AND IS NOT OUT WHILE RECUP ERATING

THIS ELECTRICAL REPAIRMAN WAS WORKING AS PART OF A THREE MAN CREW REMOVING A LARGE BREAKER FROM A HOISTING CIRCUIT. HE STATES THAT HE DEVELOPED A DULL PAIN IN HIS SHOULDER THAT GOT WORSE. DR. PRESCRIBED PHYSICAL THERAPY. DR. VISIT 12-18-00.
EE WAS LYING ON HIS BACK DRILLING HOLES IN THE FLOOR BOARD OF A WELDING TRUCK TO MOUNT A HEATERUNIT WHEN A PIECE OF METAL FELL BEHIND HIS SAFETY GLASSES & ENTERED HIS RIGHT EYE.

A ROCK FELL OFF THE HIGHWALL AND STRUCK EE'S RIGHT ARM.
THREE EMPLOYEES WERE SENT TO CHANGE A BROKEN SCREEN, INJURED WAS CUTTING A PIECE OF METAL OFF THAT IS USED TO DIVERT MATERIAL TO A CHUTE. AFTER THE METAL WAS CUT LOOSE, HE PROCEEDED TO MOVE TH E METAL OUT OF THE WAY WHEN THE INJURY OCCURRED.
WHILE MOVING BELT STRUCTURE FROM B7 BELT LINE A PIECE OF RIB ROLLED OUT BETWEEN TIMBERS POST WHICH WAS ON 5' CONTER & SLID
EE WAS CLEANING TRUCK DUMP AREA WITH A DOZER. WHILE OPERATING THE DOZER HE BACKED INTO A LARGEROCK, GIVING HIM A JOLT TO THE LOWER BACK.
LIFTED AND PLACED 50# BOXS OF WELDING ROD IN CABINET AND FELT TWINGE IN BACK.
PLASTERING STOPPING WHEN FALL ON LONGWALL OCCURRED CAUSING BLOW OUT OF STOPPING BY AIR DISPLACEMNT. INJURED KNOCKED BACKWARDS BY STOPPING BLOCK AND WAS KNOCKED DOWN TO MINE FLOOR AND STRUCK LE FT ARM ON BOTTOM.
WHILE INSTALLING ROOF BOLT A PIECE OF DRAW ROCK FELL STRIKING HIM IN THE LOWER BACK.
EE SPILLED K1 KEROSENE ON HIS PANT LEG THEN WAS GOING TO LIGHT THE PROPANE TORCH AND A SPARK IGNITED AND BURNED HIS LEFT LEG.
4 WEST LW PULLOUT - WHILE BLOWING DOWN A SHIELD THAT HAD BEED LOADED ON A SHIELD CARRIER A RAM EXTENSION BAR FELL OFF OF THE PONTOON AND STRUCK THE TOP OF HIS RIGHT HAND THAT POSITIONED ON THE CARRIER.
EMPLOYEE WAS ON HIS KNEES HANDLING THE SCALE WEIGHT CHAIN FROM #8 BELT TO ANOTHER EMPLOYEE, WHEN HE FELT A PAIN IN HIS LEFT KNEE.
EE WAS CLIMBING DOWN THE STAIRS OF A SLURRY TRUCK WHEN HIS LEFT HEEL CAUGHT ON THE BOTTOM RUNG OF THE STEPS. THIS CAUSED HIM TO STUMBLE & FALL LANDING ON HIS LEFT LEG/FOOT. RESULT: SPRAINED LEFT KNEE
EE WAS LYING ON HIS BACK INSTALLING A FUEL TANK ON A DIESEL VEHICLE. WHILE TWISTING A WRENCH HE SUFFERED A STRIAN TO HIS RIGHT SHOULDER.
EE WAS LOADING OXYGEN BOTTLE ONTO MECHANIC'S TRUCK. THE BOTTLE SLIPPED AND LANDED ON TOP OF HIS LEFT FOOT. HE SUFFERED A BROKEN BONE ON THE ARCH OF HIS FOOT.
INJURED FELT A PAIN IN HER BACK WHILE BENDING OVER TO LIFT A CINDER BLOCK.
EE WAS CARRYING ROCK DUST WHEN HE SAID HE HURT HIS BACK
PIECE ROCK FELL ONTO LEFT WRIST & HAND WHILE EE PUTTING UP BOLT.
EMPLOYEE WAS PULLING CABLE OFF SHUTTLE CAR REEL WHEN HE FELT PAIN IN HIS CHEST.
EE WAS GOING OUT THE DOOR AND SLIPPED ON THE IRON GRATE THAT HAD A PIECE STICKING UP ON IT.
EE SAID HE LEFT KNEE WAS SWELL FROM CRAWLING IN MINES. HE WAS OFF WORK F4ROM 12-12-2000 TO 12-15-2000.
EE WAS EXITING STOCKPILE TRUCK AND THOUGHT THERE WAS ANOTHER STEP. HE FELL ON HIS SIDE BRUISING HIS HIP, ELBOW, AND SHOULDER.
LIFTING BATTERIES OUT OF TRAY OF PERSONNEL CARRIER. LEFT WORK ON 12-8-00 TO GET HIS INJURY CHECKED OUT.
EE ALLEGES HE STEPPED ON CATWALK PLATFORM AND A PIECE GAVE AWAY AND HE FELL TO THE GROUND LANDING ON HIS FEET. THEN FALLING BACKWARDS FROM PLATFORM APPROX 10"" OFF THE GROUND.
EE WAS PULLING A START UP CORD FOR THE LEAF BLOWER. HE STATED THAT HE FELT HIS LOWER BACK TWIST.
EE WAS HELPING TO ATTACH A FLAT CAR TO A MOTOR. THE ENDLOADER OPERATOR WAS PSOITIONING THE FLAT CAR SO THAT EE COULD INSERT A PIN INTO THE HITCH. THE FLAT CAR SLIPPED, CRUSHING EE'S LEFT MIDDL E FINGER.
AS EMPLOYEE WAS WALKING DOWN A HILL HE SLIPPED &FELL ON SNOW & ICE, LANDING ON HIS BACK, CAUSINGA SPRAIN. HE CONTINUED TO WORK THE REST OF THE WEEK. HOWEVER, ON SATURDAY, 12/09/00, HE SOUGHT MEDICAL TREATMENT & WAS AUTHORIZED OFF WORK FOR ONE WEEK.
LOADER OPERATOR PICKED UP LARGE ROCK DROPPED INTO BED OF THE ROCK TRUCK INJURED OPERATOR, WAS DRIVING AT THE TIME OF THE ACCIDENT.
EE WAS LIFTING A BOARD OVER HIS HEAD WHILE HE WAS IN A SITTING POSITION. HE WXPERIENCED BACK PAIN AND WAS UNABLE TO RETRUN TO HIS NEXT SCHEDULED SHIFT. ON JAN 8, 2001 SMC RECEIVED A MEDICAL OP ION THAT THE INJURY WAS NOT A PRE EXISTING CONDITION, WHICH MAKES IT A REPORTABLE, LOST TIME ACCIDNT.
ROCK FELL FROM BETWEEN THE SHIELDS, STRIKING EE ON THE LEFT HAND, CUTTING HIS RING AND MIDDLE FINGER. EE IS LOST TIME INJURY.

EE ALLEGES WHILE WALKING TO HIS TRUCK HIS LEGS AND FEET SLIPPED FROM UNDER HIM, THE GROUND WAS COVERED WITH ICE AND HE FELL ON HIS RIGHT ARM AND SHOULDER.
EE WAS SCRAPING ROCK MATERIAL OFF OF CATWALK WITH SHOVEL RESULTING IN STAIN OF RIGHT LOWER FOREARM/WRIST.
