Accident type, without injuries

An non-injury roof fall was discovered near the No.14 Reams Lick Seal. Fall measured approximately 50 feet long x 20 feet wide x 4 feet high. Seal was not damaged. A clean up plan was submitted to MSHA.

Operator was backing down EE's ramp, a Hyd. hose must have broke close to the turbo on the engine causing a fire. operator shut the dozer off and hit the fire suppression system and exited the dozer unharmed.

An unplanned roof fall occurred in the #11 3rd main east in the #11 entry at spad 29+20. The fall is approximately 20' wide and 30' long and around 8'thick.

An unplanned roof fall occurred on the haul road of the S3-B between crosscut 35 and 36. The fall measured 36' in length, 15' wide and approximately 7' thick.

Mucker was doing a final back drag of the heading when they a small amount of fly ruck and dust. Backed the mucker out of intersection. Walked in and found an exploded booster. Secured scene.

#3 hoist stopped due to a brake fault. The solenoid connector shorted out on one of the brake manifolds. Upon investigation it was found that in the PLC board, a rack output had shorted out which led to the shorted out solenoid connector.

Contractor maintenance work (installing a sheet metal patch) was being performed on a bin under a vibrating screen at the facility wash plant. Welding slag blew through exterior wall of bin, igniting combustible rubber liners inside bin, causing a fire which impacted the bin, a vibrating screen, four conveyor belts, and adjacent infrastructure.

At 3:40am a roof fall was encountered on the 1 south section. Fall was inby the tailpiece. Location of the fall A entry @ 0+59 to 1+20. Fall is approximately 60 feet long,18 feet wide,by 12 feet high.

While performing an inspection in an old heading, it was found that a fall of ground had occurred.

While performing an inspection of the mine, the supervisor came across a fall of ground.

A breaker for the battery backup for the #2 hoist tripped during a severe weather event during the night. This caused the batteries to wear down because they could not charge due to the tripped breaker. Once the battery level got low enough, it tripped another breaker to the Grinnell System. This caused the hoist to shut down.

At approx. 6:20 PM the control room operator got an alarm that the coal belt shut down. The operator sent the utilitymen to investigate what the cause was. At approx. 6:30 PM the utilitymen determined a fire was in the #2 coal feeder. The fire department was called and clay was put on the feeder and the fire was smothered around 1:30 AM.

While Sandvik 421i closed cab double boom jumbo was drilling high bottom in the portal 1 decline there was an unintended initiation of explosive from a miss hole that was not identified during cleanup and inspection of the area. No injuries or Equipment damage was found.

There was an unplanned roof fall at crosscut 50 in the #3 8th South Panel in #6 entry.

At 2:45 pm on 5/17/2023 the primary escapeway elevator was taken out of service. It was determined a hydraulic valve had failed causing the brake to lock. MSHA was notified, underground production was stopped, and all miners were evacuated to the surface using the secondary escapeway. MSHA was notified that the elevator was back in service on 5/18/2023.

During a thunderstorm, Turquoise Ridge experienced a power bump. When electricians reset the power, the hoist computer screen did not come back up causing the computer to lose its settings. The setting needed to be inputted manually.

A roof fall occurred in the intersection of Entry #5 at crosscut #16 on the 3rd Main West Travelroad. Entry #5 is designated as the Secondary Escapeway.

There was an unplanned roof fall in the #9 1st south corridor in the #4 entry at spad 52+90.

An electrician was working up in a manlift basket that would not return to the ground. The Field PCS and the electrician on the ground attempted to bring the lift down on battery power it would not work. After verifying it is safe to jump the lift battery, the electrician was safely brought to the ground under the lifts power. It took about one hour.

A lightning strike caused one of our main Avista (Power Company) transformer to break down. We lost all power to the site and could not operate the Silver Shaft Hoist.

A hot spot on a coal pile was discovered at the Prairie Eagle East Portal Raw coal pad. No one was injured. Steps are being taken to transport the hot material to the prep plant to be washed and taken from property.

The door switch on the # 9 elevator quit working, a new switch was installed.

Hoist number 2 went down for an under voltage card issue. Hoist #1 was still out of commission waiting on an encoder to be replaced. At 5:00 P.M. we evacuated the mine due to two of our 3 conveyances being out of service leaving us only 3rd shaft. All miners were out of the mine and accounted for at 5:51 P.M. The hoist came back up at 6:40 P.M.

While dropping the hoist car into the mine, damage occurred to the hoist car axle that caused the car to de-rail. The hoist car was taken out of service while the slope track was examined for derailment issues and the brake car's axle was changed. No track issues were identified.

A NON injury roof fall occurred @ Cross Cut 17 in the #2 entry (Belt Line) on the Mudlick Mains. After investigation,it was determined that the cause of the fall was due to age deterioration of the area.

Encoder malfunctioned causing hoist to not function at 100 percent capacity. Hoist was downed until encoder can be replaced.

Employee received fatal injuries while operating haul truck which over-turned backwards off a stockpile.

While SC0539 Bulldozer was traveling on 1B Ramp, a large volume of material entered the road and contacted the bulldozer.

Miner was found to be ran over by a stacker while crushing at our East Pit. Miner was deceased when found by other miners.

Teammate while holding the Manual override with their right hand inserted their left index finger into the Borax Valve to see if it was clean. The teammates stated their right hand slipped off the controls and the valve closed and amputated approx 1"" of EE's left index finger.

While swinging bolter head, employee tripped and fell forward and put right handout to catch their fall and left hand grabbed joystick which dropped bolter head onto their right hand causing 2 fingertip amputations.

Struck by flying object

A section of pipe was being removed from the plant dredge. The pontoons have a hook for a sheave that holds the electric cable. They had the cable removed and hung 2 sheaves on the pontoon hook. When hitting the pontoon w/ the workboat to separate the pipe and rubber elbow one of the sheaves flew off crushing employee's left thumb while they sat in the workboat.

IW was hooking up a small water trailer to a pickup truck. In the process EE fell and reached out to catch self. EE's left pointer finger tip was caught in between the trailer hitch and ball. Approximately 1/2"" of the tip was amputated. Taken to ER where they performed surgery to properly close the wound.

EE was closing the window on equipment. They caught their right pinky finger in the window, and amputated the tip of the pinky. Seen at ER, consulted with hand specialist at UofU, and referred to them for treatment. Seen on 5/10 and kept off work pending follow-up on 5/31. Pinky amputated to first distal knuckle. 50 lost days.

Worker was leaving at the end of the shift & saw a pin on an excavator track sticking out. They & another worker tried to reinstall pin with the forks of a skidsteer & the worker got a finger pinched between the fork & the track, causing partial amputation of finger.

Working on log wash and hand got caught in between log washer and the frame.

Worker was removing a rock from an airlock when the rock slipped and smashed their left middle finger between the rock and the housing. This caused a fracture and laceration to the employee's finger. Employee was placed on modified duty.

The workers were lining up some chutes, they were lifting them up with picking eyes, as they were hoisting the steel plate it got caught with other steel, when they were about to loosen the hoist the plate got loose from picking eye and dropped, nobody was underneath the steel plate but it bounced from another steel and it just caught one of our workers

Injured employee was checking engine fluids and fell.

The employee was sitting in the deck of a pinner moving it. The employee was holding to the top of the canopy with left hand, the canopy came into contact with the top mashing two fingers

A fully trained miner was installing a slider tube when they felt pain in their back. The injury was not the result of any compliance issues nor were any protective guards involved.

After installing new motor on roto packer and being down on knees for 45 min to complete installation and wiring, employee stood up and heard knee pop. Became reportable in September 2024 due to surgery.

Employee alleges that while drilling an angle to pass a screw, the continuous drill entangled the little finger of EE's right hand with the cable.

The employee stated : was putting up a piece of hog paneling when a rock fell out hitting employee in the nose

The employee was installing a roof bolt in the #5 entry, a piece of draw rock fell hitting EE's left ankle.

A fully trained miner while roof bolting stepped on a piece of coal and said knee popped. The injury was not the result of any compliance issues nor were any protective guards involved.

Fall from ladders

Miner fell from a ladder approx. 4 to 5 feet as EE was descending. Miner landed on a concrete floor. This resulted in muscle soreness and a concussion. Lost time began on 06-02-2023

Employee was putting grease on jack eye when jack slipped and pinched EE's finger in between jack and pin base.

Temporary staffed labor (C2605) was pumping diesel fuel when they used a knife to cut away an obstruction. The knife slipped cutting their left palm.

Over-exertion in lifting objects

IE was raising up a high voltage cable over EE's head when EE felt a pain in EE's right shoulder. IE was taken off work for surgery on 9/25/2023. IE had surgery to repair a torn rotator cuff on EE's right shoulder.

Fall onto or against objects

Worker slipped due to the muddy conditions and hit forehead on the end of a steel bar as EE fell. Deep laceration required stitches at a clinic.

Struck against stationary object

Employee was driving a motor carrying an empty trip. EE slowed down suddenly to check a switch and the trailing motor slid into the empty trip causing EE to hit EE's right shoulder inside the operators compartment.

Struck by falling object

A concrete slab was being placed onto a structure by crane, and the employee was using a shovel to help guide it into place. As the crane operator set down the slab onto the structure, the slab broke apart. A piece of the slab struck the employee on EE's upper back knocking EE to the ground.

EE was cranking a come-along that was attached to a floor panel. The floor panel broke in the Dryer and the come-along hook came back hitting EE in the mouth. Stitches, Broken Teeth and Fracture to bone in mouth.

Employee was getting back in the cab when felt their knee pop as they stepped down into the cab of Water Truck 24. The employee reported swelling and pain.

Employee was walking at x-cut 59 on the 12 section, when a piece of brow fell out striking employee on the left foot causing laceration to their foot resulting in sutures.

Employee was prepping to shear the fingers of a finger splice. EE started to shear the fingers with an oscillating tool and slipped with the shears causing the shears to strike right pinky finger.

A fully trained miner was going through a mandoor when the door shut on the miner's leg. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner injured finger when the miner dropped a rock on it. The injury was not the result of any compliance issues nor were any protective guards involved.

Fall from ladders

Employee was on a 6 foot step ladder getting ready to wedge the wall which was pyramided up in the middle. As employee was attempting to wedge the top of the wall, the wall started to fall toward them. The employee jumped from the ladder, landing on the pavement, injuring left ankle.

EE TRYING TO STRAIGHTEN BUMPER ON TR-11 USING CHAIN, 20TON HYDRAULIC JACK. EE USING ROSE BUD TRYING TO HEAT REAR BUMPER TO STRAIGHTEN. CHAIN UNDER TENSION BROKE, CHAIN HIT EE IN FACE/MOUTH. EE FELL BACKWARDS LOST CONSCIOUSNESS, STRUCK BACK OF HEAD.

Employee was pulling an old roller off of the beltline and felt a pain in lower back. EE continued to work until the doctor took EE off of work on 06/01/2023.

Employee states was pushing bolt to top when rotation hose got caught under motor at worm gear and pulled hose loose from fitting causing hose to strike operators hand resulting in 2 fractures to middle finger.

EE was ascending a ladder on a Haul Truck when EE felt a pop in their shoulder/arm. EE was evaluated by an Orthopedic, and was determined that EE tore the rotator cuff.

Miner carrying starter up stairs to change starter on crusher engine. Miner tripped on stairs with starter in EE's hands and starter fell on EE's thumb and smashed it.

As the Excavator operator started to access steps to enter cab, the operator felt dizzy. Operator stepped back to ground level and leaned on tracks to regain stability. At that time, the operator was taken to the hospital for observation which resulted in a Medical Complication (low Blood Pressure).

Employee was manually moving the position of tripper 3 with another employee when they felt a strain around the Achilles of their right foot.

Employee states they hit a pothole causing them to injury their back.

Miner was standing on a plank and plank slid.

Employee was using a water hose to clean at the 4 West #1 Belt Drive motor. When EE turned the water on, the pressure caused EE to lose balance and fall down. EE landed on the left knee, hitting it on the concrete pad/metal drive motor base. EE was wearing required PPE, has training and experience in job. No rules or regulations were violated.

Employee was helping get a mantrip through a track switch on the surface when the mantrip struck employee's right foot causing employee to fall and hurt left knee. On 6/28/2023 the operator received the report of an MRI showing a non-displaced fracture of the patella. This is now being reported as an NFDL incident.

At the face, Belting up. EE was pulling low structure over. While doing that, EE felt a pain in the stomach area. Later to find out EE got a Hernia.

Truck driver stepped down off the low boy trailer and stepped on to some chains that they had thrown off the trailer and twisted an ankle.

Struck by rolling or sliding object

Employee was working on mantrip in the top shop. When kneeling and removing the u-joint bolts, the axle shifted and rolled off the forks, pinning the employee's arm to the floor. This resulted in injury to EE's right arm, wrist and hand.

Struck by flying object

Was measuring a hole to see if it was ready to be stemmed. The weight on the end of the tape got hung up. Employee pulled on the tape causing it fly out of the hole at a high velocity and strike the employee in the groin area.

An employee was sweeping the floor in the LO1432 Loader at the end of the shift. When EE straightened their back, EE noticed pain in the lower back.

While operating a 988K Wheeled Loader (Co # SL22), during normal working tasks, Employee stated was experiencing back pain with pain to the right leg. Employee indicated that the pain had become more intense throughout the shift.

Employee is an Equip Opr./Quarry Truck Driver. EE was operating a Quarry Truck, EE was waiting for EE's truck to be loaded when a rock ""spilled"" or ""fell"" from the Bucket of the WA-900 Komatsu Mach which was loading EE's truck, broke the driver's side window, hit EE on the L/thigh and fell to the floor of the truck EE was operating.

Employee was helping carry press belts to the belt line when employee on the opposite side dropped their side causing plates to hit employee's right knee. Employee started missing work on 5/30/2023.

Employee installed the outside bolt on the 4th row in the crosscut, while holding the steel EE dropped the pot to swing it in. As the pot dropped it came in contact with the steel EE was holding causing the 4 foot drill steel to penetrate EE's right boot resulting in a fracture to a toe and laceration to the foot

While tightening a ratchet strap the employee felt a pain in their right bicep. A torn bicep was confirmed and the employee was taken off from work.

Employee stated was lifting a 5 gallon can of hydraulic oil and the can slipped off car pulling employee's arm downward.

EMPLOYEE WAS DRIVING A JOHN DEERE TRACTOR HAULING A TRAILER WITH SUPER SACKS OF SALT. THEY STOPPED TO UNLOAD THE BAGS AND ROLLED AN ANKLE WHILE STEPPING OFF THE TRACTOR.

EE was cutting a bolt head off a plate that was secured in a vice. EE was using a portable circular saw. Saw blade contact the EE's left middle finger resulting in a laceration.

Lifting Screen Mesh onto screen deck with service truck. Screen hit conveyor and dropped hitting employee across the forehead. Hook on auto crane was in place and functioning.

Employee was changing brake pads on a haul truck. As the employee lifted the caliper up EE attempted to slide a jackstand under it. EE's hand slipped off of the caliper and it dropped catching right ring finger between the jackstand and caliper. Employee suffered a broken right ring finger.

An employee tripped while mounting a haul truck. EE felt pain in the shoulder as EE caught self. The employee was evaluated at the clinic and later (on 6/3/23) received medical treatment for the injury, making this reportable. The employee was not released to return to work.

An employee's left hand became lodged between the rotating pulley and the gear box shaft on radial stacker

Struck against a moving object

While riding out of the mine on 5/21/23 employee stated was jarred in the seat resulting in back pain. EE did not report this to management. On 5/24/23 while on days off EE reported to the supervisor that EE was seeing a doctor for continued back pain resulting from the incident on 5/21/23. Restrictions were issued limiting EE from performing all required job duties.

Employee states the rotary production drill stopped suddenly while tramming to the next hole causing back injury.

Struck by falling object

Employee was tramming the left bolter from the inch tram control to install the last row of bolts when a piece of rock measuring 13 1/2' fell and struck the employee on the left arm and hip causing a bruise and scrape. EE went to Dr on 5-22 and Dr gave EE 2 days off work.

Employee stated was operating the 20-ton motor and had left hand on the wind shield and hit a kink and employee's hand slipped off wind shield and cut hand.

IE was carrying oxygen tanks to strap on the rib and a fellow employee dropped EE's tank on IE's left foot. IE was diagnosed with a bruised left foot.

A fully trained miner was standing in front of a scoop when the hanging bolter cable came down causing EE to hit EE's head on the scoop. The injury was not the result of any compliance issues nor were any protective guards involved.

(New Construction) While installing chute work, employee ascended extension ladder placing self in a pinch point area and the pathway of plate work chute being hoisted by use of chain hoist causing injury to left jaw

Employee strained left knee while installing a 54"" bottom belt roller.

Absorption of radiations, caustics, toxic and noxious substances

WHILE EMPLOYEE WAS REINSTALLING THE LIDS ON A SCREW, THE BUELLS ABOVE LET GO AND DUMPED HOT DUST WHICH SPLASHED ON THE EMPLOYEE. THIS KNOCKED EE DOWN, AND EE CRAWLED OUT OF THE AREA. EE RECIEVED 3RD DEGREE BURNS ON SEVERAL PLACES ON EE'S BODY.

IE was walking down the slope when IE slipped. When IE slipped IE rolled an ankle. IE finished shift prior to requesting medical treatment.

Over-exertion in pulling or pushing objects

IE was dragging a hose up the slope to wash. The hose caught and instead of going to release the hose the employee decided to try and jerk the hose free. When the employee jerked the hose it stretched and then it pulled back straining IE's shoulder.

IE hit EE's finger with a hammer when capping in a crib tie causing a fracture.

Employee was riding in a light vehicle coming out of the mine when a foreign body entered left eye. Employee washed it and saw site medic, was referred to an eye specialist, they were diagnosed with a rust ring starting and had it removed and was given restrictions for 3 days.

IE was building a kennedy wall when back started hurting. Dr. told IE to take some days off.

Employee stated was installing a bolt when a rock fell and smashed left hand on drill mast.

Employee was cleaning out the loader bucket with a scraper. EE jarred right shoulder, but did not seek any treatment until December 1. EE went to a clinic, and they suggested physical therapy, which helped. EE still had pain, so EE got an x-ray and MRI. It was then discovered EE a had a torn rotator cuff. EE is scheduled to see an ortho surgeon for further evaluation.

A fully trained miner was walking upstairs when the miner fell and injured ankle. The injury was not the result of any compliance issues nor were any protective guards involved.

The EE was carrying Cement blocks, when EE tripped over the plastic banding that secures the bundle of blocks. The EE caught self before falling to the ground and felt a pain in lower back. The EE was wearing PPE. Started missing work 5-18-23, Returned 5-23-23.

The EE was making a cable splice on the FCT, when the blade slipped causing EE to cut left knee with the utility knife. The EE was wearing all PPE. Began missing work 5-18-23, Returned 5-22-23

Employee was sitting in a parked mantrip on 2 East Track 57 wall when it was stuck by unattended slag cars. The Employee received 5 staples to the head and cracked spinous process C7 vertebrae. No other part 50.20-5 criteria apply.

Employee was walking through crosscut slipped on a small ledge and felt a pain in right knee.

Employee was stung by bees during outdoor housekeeping tasks.

Fall onto or against objects

Employee was in the work boat and working on the back of the dredge. The employee tried stepping on to the back of the dredge hitting EE's head on the exhaust pipe attached to the dredge motor. This results in the employee falling back into the boat striking EE's rear end on two guide bolts.

EE was attempting to remove a loose rock that was laying on top of EE's canopy when the rock slid off striking EE in the left shoulder. X-rays were negative. EE was taken off work starting on 5/17/2023 for a contusion to EE's shoulder.

While tightening a bolt on the crusher wear plates, the employees gloved hand slipped off the ratchet EE was using. EE fell back onto the other plates, fracturing EE's left femur.

GMS Employee was a passenger in a mantrip that was traveling in between a set of airlock doors when the mantrip was accidentally struck by slag cars causing injury to the employee.

Over-exertion in pulling or pushing objects

Injured employee was bolting off of the miner. While pushing a roof bolt up into the bolt hole, employee felt pain in EE's right shoulder. No rules or regulations were violated and employee has training.

Struck by falling object

While checking a splice on EE's shuttle car cable, a piece of rock fell from above the coal seam that was on the rib/mine roof and struck the employee's left leg causing a fracture to EE's tibia. EE began missing work on 5/17/2023.

Struck against a moving object

Employee was sitting in a parked mantrip on 2 East Track 57 wall when it was struck by unattended slag cars. The Employee received 3 staples to the scalp. No other part 50.20-5 criteria apply.

A gravel truck hauling sandstone from the mine site ran into the ditch line while attempting to pass an empty coal truck coming on to the mine site. The accident resulted in deep bruising to the Mar-kel truck driver and lost work days.

IP was using chain hoist to raise sump pump when the hoist stopped. IP jerked on the chain and felt a pain in shoulder. IP continued to work the remainder of the shift and the following day. Injury was reported to operator on 5/17/23 and accepted by WC on 5/30/23.

The EE was in the process of securing a belt scrapper with a yellow rope hanger. The hanger caught something on the EE pulling their right arm into the belt roller. The EE was wearing the appropriate PPE. The EE began missing on 5/16/23.

Upon a follow-up appointment, employee was referred for an MRI. On 5/31/2023, MRI results indicated a torn triceps tendon at right elbow. Employee was referred for surgery, has not yet been scheduled.

The employee reports today, june 16, 2023, that on Monday, June 12, 2023, while working on the installation of a water pipe on the Cantera property there was a shot to extract material and that with the wind the dust reached the area EE was working and fell into EE's throat. EE indicates that at that time of the incident was not wearing mask.

The employee was mixing Nitrate and Diesel fuel in the Bulk Truck to produce Ammonia Nitrate. As the Nitrate slid down the bin, the flow of Nitrate stopped because of a crust build up. The employee used a digging bar to break the crust to resume flow. When the bar broke through the crust, the employee fell forward striking the lip of the bin.

Employee was walking in the No.4 to No.5 crosscut and tripped over the C/M cable and fell injuring left and right knees.

Employee placed foot inside pallet while stacking bags. Coworker lowered the pallet lift, catching the employee's foot. Employee bruised foot and was prescribed ice and NSAID's. Resulted in LTA for facility. Employee returned to work on 5/16/23 with full medical clearance.

Employee was collecting blast hole samples in the East Pit on 5/11/23. The employee reported pain in their lower back during sample collection. On 5/16/23, the employee notified their direct manager that they were not reporting to shift due to fatigue, which they attributed to back pain.

Mechanic was removing a head pulley from conveyor pushing it by hand off the end of frame work and into loader bucket. Mechanic's hand got caught between loader bucket and head pulley resulting in multiple broke bones in left hand. Loader was stationary the entire time.

The miner was using the skid steer to load up trash and debris left on shore from the dredge to take to the dumpster. Miner had feet propped up on the skid steer steps, and simultaneously lowered the bucket which came down on miner's foot. Minor injuries (foot contusion) was sustained.

Employee was rib bolting off the miner at approximately 24+40 survey station. Upper portion of the rib became dislodged striking the legs of the employee, as a result they fell hitting their head on the miner. The rock measured 58"" long x 30"" high x 6"" thick. EE suffered a hematoma and concussion like symptoms. No compliance issues were found. No other part 50 criteria apply.

The miner was removing a section of an elevated walkway when the walkway beneath EE fell. The miner fell approximately 18 feet to the concrete pad below. The miner received injuries to the ribs, elbow, kidney and spleen. The miner is now nearing full recovery from injuries, but has not yet returned to work.

Setting up the plant, operator of end loader had placed the conveyor hopper on blocks. They backed up and the chain hook caught the conveyor/hopper and dropped on injured employee's foot. EE had steeled toe boots on and got no broken bones, but the toes were pretty squashed. EE had stitches and developed compartment syndrome in their calf.

Employee was dismounting a rock truck. Employee stated they looked over at another piece of equipment for their lunch box and missed a step losing balance. Resulted in the employee falling to the ground and injuring right ankle/shin.

Drill steel came out of the carriage and fell in to the housing. We then laid the tower down to maneuver drill steel back in to place. While doing this the drill steel kicked back knocking EE off the drill rig on to the ground. EE sustained a nose bleed and back pain. We then called an ambulance and EE was transported to a hospital.

Injured was an employee riding in a track mounted man-trip, when it derailed and struck the coal rib causing the man-trip to stop suddenly. The employee was thrown into the man-trip, and EE suffered a laceration that required one stitch and blunt trauma injury to EE's right eye. EE was wearing safety glasses at the time.

Employee went to a follow up appointment on 09/26/23 where EE was advised that the MRI that was given on 08/16/23 showed a labral tear of the right shoulder that will require surgical repair in the future.

A shuttle car operator got stuck. A battery scoop operator pulled it out, the car rolled back down, they started the car and backed up to give chain slack. They bumped the scoop operator with the car on the upper shins. The Maintenance manager took them to the doctor. the X Rays taken revealed that there was a small hairline fracture in the upper right shin.

Employee was lining up a jack pad on undercutter #2. The steel slipped when attempting to line up the jack pad and pinched EE's middle-left finger between steel frame and pad.

Employee was setting a Heitzman jack when a piece of roof rock fell measuring 3 feet x 2 feet x 8 inches thick. The rock hit employee's right foot.

Employee sought medical attention and was diagnosed with a transverse fracture of the right metatarsal bone in the right foot. Employee was given a full release and returned to work on next regular scheduled shift (5/8/2023).

While checking the torch on K1, employee noticed an air lance inserted into the cyclone on K2 hood and when walking to check why it was there the material blew out of the pipe and onto their arm and face.

Employee was attempting to cross the belt line and had one leg crossed over and they under estimated the distance to the mine floor while the other leg was still on the belt and their leg impacted the metal belt rail with all of their weight on that leg as their leg slid down the rail and this resulted in a torn muscle on the inside of the leg.

Driving a forklift underground on hit a few bumps and got a sore back and no pain that day. on 5-7-23 their back started hurting so they got it checked out at the ER. The doctor let them finish the shift and see a ortho if not better in 2 weeks. on 5-31-23 they got results and got an no work release.

IE stood on a belt conveyor to inspect a stone feeder that may have been leaking. IE did not lock out the belt prior to accessing it. Conveyor belt started up and pulled the IE through a 14' height clearance before IE was able to pull the E-stop cable and climb off the belt. Incident caused multiple fractures in employee's back requiring EE to have surgery.

An employee was injured when a hammer fell through the collar doors as they opened. The employee was on the work deck cleaning up after a concrete pour. The hammer struck the employee on the right shoulder causing a laceration and fracture.

The employee was installing new buckets on FM#3 discharge elevator. The bucket weighted roughly 25lbs and was lifting the bucket at chest level. Employee has been doing physical therapy from 5/25/2023 until 1/31/24. The doctors diagnosed EE with a torn rotator cuff and went into surgery 1/31/24.

EE was mounting a haul truck wheel. Stood tire and wheel up to sit the lock ring, turned to walk away to continue working, and felt the rim fall on them. Initial report is that the rim ""popped"" out. Investigation is ongoing. EE transported to hospital, diagnosed with multiple fractures, and transferred for surgery. Surgery took place approximately 10 am on 5/24.

Employee advised Manager that they bumped their right knee because they could not move the seat. Manager got in the haul truck and the seat did move back. EE reported the knee was fine, then on Saturday the 27th sent a text stating they wanted to go to the doctor on Tuesday, May 30th. They had just been released to full duty 5/15/23 from a left knee injury.

Employee reports injury while descending steps on the M/V White Rock. As the deckhand was going down the Engine Room steps, employee slipped on the second step from the floor and attempting to stop the fall injured right shoulder. Employee also suffered a small avulsion on right index finger.

Employee was trying to loosen a bolt on a continuous miner lid when they felt a pop and pain in left bicep. Employee worked until scheduled surgery on June 13, 2023.

IP was being trained in the use of a hose making machine when the trainer was demonstrating how to place dies. While in the process of closing down on the dies, an object struck the IP in the right shoulder.

As the employee lifted the drill steel with the drills jib the steel got caught on the drill head, causing the bottom of the drill steel to kick towards the drill rig. When the drill steel kicked towards the drill the employee put EE's arms out to redirect the drill steel into place and from hitting the drill. The pipe hit EE's forearm against the metal frame of the safety gate.

Miner was negotiating stairs to perform maintenance on a screen. When they turned to face screen, their foot slipped off of the stair tread causing them to trip down the remaining two steps and sit down on top of their right foot/ankle. Miner was evaluated for right ankle strain.

Injury was reported to employer on June 20, 2023 at 12:30 PM. Employee states, in mid-May IE was helping another EE change the top screen cloths on an 8x20' middle deck. The other EE cut the screen in 1/2 while IE was pulling up on the half screen. While pulling up IE states IE's neck and spine began to hurt.

Miner finished welding under the primary jaw crusher while wearing a welding/grinding hood. They switched to a face shield and used a wire wheel to clean the welds. They removed the shield, put on their safety glasses, and began moving the tools out from under the jaw crusher. Wind moved some of the dust as they climbed out. They then felt something in their left eye.

Employee was loading a broken heel plate from a loader into the back of a service truck. EE cut left pinky finger on the plate as EE tossed it into the bed of the truck. EE received 8 sutures and was released to full duty.

Employee had hand on canopy of underground forklift while operating. Canopy contacted mine rib pinching left hand. Resulted in a laceration requiring 13 sutures.

EE was working Saturday when leaving the maintenance trailer, EE was locking the door when a bee stung EE just below the left eye, and two more bees stung EE on either side of the bridge of EE's nose. EE ran to the loading dock area, talked to another EE, clocked out at the loading dock, then made EE's way to EE's truck. By the time EE got home EE's left eye was swollen.

On 5/11/23, operator was using a dolly to move ten-50lb bags of material. Operator pulled dolly back, lost their footing and caused the momentum of the material to push the operator down and they fell on their left knee. The operator was treated with first aid for knee abrasion. Operator did not want to see a doctor when injury occurred. Operator requested to see a doctor 5/19/23.

While cutting the sweep, employee was not wearing cutting gloves and overrode self-retracting action of knife. An employee was in the process of breaking down cardboard boxes for recycling. Employee slid a bare hand across the box flap contacting a 3"" staple that entered the bottom and exited the top of the left-hand index finger. Staple was removed by the PA, and the employee returned to work without any restrictions.

EE was lifting an empty pallet that was caked with product residue. The pallet was heavier than originally thought. EE dropped the pallet hitting their fingers.

Did not receive any medical treatment until 6/23/2023 when EE started therapy.

Mechanic removed cutting edge from loader bucket. Following removal and placement of old cutting edge inside bucket, they pushed the cutting edge out of the way. In doing so, the edge lacerated glove through to left index fingertip. 4 stitches required.

Miner was mucking the face after blast and hit a toe in the sill which caused the mucker to stop abruptly, miner hit their head in the cab.

On 5/12/23 after pulling open a sand box truck lid an employee noticed an abdominal protrusion. On 7/20/23 the employee was diagnosed with an abdominal hernia. Surgery to repair the hernia is scheduled for 8/3/23.

A Laron employee on upper end of the #2 Apron Feeder repositioned their body, laid down a sleever bar employee was using on the structure when it began to fall approximately 25 feet toward a WIRC employee striking the ear on the right side of the head lacerating the ear.

While changing a fuel transfer pump, technician went to clean off oil pump hardline with Brake Kleen, sprayed above EE's head but the nozzle was facing EE instead of away. Brake Kleen hit EE in the eyebrow and ran down under EE's safety glasses into EE's eye. Immediately flushed eye out at the eye wash station, then flushed again with saline solution.

I.E. was driving roofbolter from cab while pulling up cable slack to last open. As they were backing around corner, they leaned back to see the light that was flagging them. As the I.E. leaned back they came in contact with the coal rib pinching the back side of their upper right arm. This caused two lacerations that required stitches. I.E. has not missed work due to the injury.

An EE was injured when they stepped back on the Roof Bolter Operator Platform, and missed footing. They fell between the platform and the TRS Scissor-lift frame. They struck the left side of their face on the frame, causing a laceration to the left side of their face. It required 3-stiches to close the laceration.

Miner was removing a snap ring from a crusher eccentric. It came apart with more force then expected and slapped the miner's finger against the wall of the eccentric. Causing laceration to the left pinky finger.

After using a metal hammer to adjust a gate, employee tossed the hammer. Unbeknownst to the employee, there was a metal ""burr"" on the hammer that cut their right index finger. Employee was taken to medical clinic for treatment. The employee's finger was treated with 10 stitches, and employee was released to return to work.

Using a hammer to put a miner chain back together when a piece of metal came off the chain and hit IE in the forehead. IE received 1 suture.

Employee was removing cutting edge from 966 loader. While removing the carriage bolts from the cutting edge, the bolt dropped down and started to turn. The employee pushed the bolt back up with right index finger, the bolt turned again cutting EE's finger requiring 7 stitches.

I.E., stated EE was replacing a collapsed tube when I.E., head was pulled up to the tube due to the suction. I. E. went to the hospital after shift and received stitches.

Deckhand was moving a cable when a burr went through their leather glove and punctured left pinky. The burr had to be removed at the hospital. Antibiotic was prescribed and tetanus shot given. Returned back to work. No other treatment given.

Operator when entering a loader, pinched finger between the door and the door frame. Operator refused treatment on first day. Went in for treatment on 5/2/2023. Diagnosed with fracture of the fingertip.

While using a hammer to hit a punch, the employee missed striking left thumb. This resulted in 9 stitches and a fractured bone at the end of the thumb.

EE was preparing to work on the panel of the right-side dump S/C. The rub rail was bent against the panel lid. EE jerked the panel lid and it dropped striking EE on the left middle finger causing a fracture and laceration.
