Mining Incidents in May 2023

An non-injury roof fall was discovered near the No.14 Reams Lick Seal. Fall measured approximately 50 feet long x 20 feet wide x 4 feet high. Seal was not damaged. A clean up plan was submitted to MSHA.

An unplanned roof fall occurred in the return No.1 Entry. The fall measured approximately 30'L X 7'H.

Operator was backing down EE's ramp, a Hyd. hose must have broke close to the turbo on the engine causing a fire. operator shut the dozer off and hit the fire suppression system and exited the dozer unharmed.

Loss of power feed from the utility company, Cleveland Public Power (CPP), that provides power to hoist.

An unplanned roof fall occurred in the #11 3rd main east in the #11 entry at spad 29+20. The fall is approximately 20' wide and 30' long and around 8'thick.

An unplanned roof fall occurred on the haul road of the S3-B between crosscut 35 and 36. The fall measured 36' in length, 15' wide and approximately 7' thick.

Mucker was doing a final back drag of the heading when they a small amount of fly ruck and dust. Backed the mucker out of intersection. Walked in and found an exploded booster. Secured scene.

#3 hoist stopped due to a brake fault. The solenoid connector shorted out on one of the brake manifolds. Upon investigation it was found that in the PLC board, a rack output had shorted out which led to the shorted out solenoid connector.

Contractor maintenance work (installing a sheet metal patch) was being performed on a bin under a vibrating screen at the facility wash plant. Welding slag blew through exterior wall of bin, igniting combustible rubber liners inside bin, causing a fire which impacted the bin, a vibrating screen, four conveyor belts, and adjacent infrastructure.

At 3:40am a roof fall was encountered on the 1 south section. Fall was inby the tailpiece. Location of the fall A entry @ 0+59 to 1+20. Fall is approximately 60 feet long,18 feet wide,by 12 feet high.

While performing an inspection in an old heading, it was found that a fall of ground had occurred.

While performing an inspection of the mine, the supervisor came across a fall of ground.

A breaker for the battery backup for the #2 hoist tripped during a severe weather event during the night. This caused the batteries to wear down because they could not charge due to the tripped breaker. Once the battery level got low enough, it tripped another breaker to the Grinnell System. This caused the hoist to shut down.

At approx. 6:20 PM the control room operator got an alarm that the coal belt shut down. The operator sent the utilitymen to investigate what the cause was. At approx. 6:30 PM the utilitymen determined a fire was in the #2 coal feeder. The fire department was called and clay was put on the feeder and the fire was smothered around 1:30 AM.

While Sandvik 421i closed cab double boom jumbo was drilling high bottom in the portal 1 decline there was an unintended initiation of explosive from a miss hole that was not identified during cleanup and inspection of the area. No injuries or Equipment damage was found.

There was an unplanned roof fall at crosscut 50 in the #3 8th South Panel in #6 entry.

At 2:45 pm on 5/17/2023 the primary escapeway elevator was taken out of service. It was determined a hydraulic valve had failed causing the brake to lock. MSHA was notified, underground production was stopped, and all miners were evacuated to the surface using the secondary escapeway. MSHA was notified that the elevator was back in service on 5/18/2023.

During a thunderstorm, Turquoise Ridge experienced a power bump. When electricians reset the power, the hoist computer screen did not come back up causing the computer to lose its settings. The setting needed to be inputted manually.

A roof fall occurred in the intersection of Entry #5 at crosscut #16 on the 3rd Main West Travelroad. Entry #5 is designated as the Secondary Escapeway.

There was an unplanned roof fall in the #9 1st south corridor in the #4 entry at spad 52+90.

An electrician was working up in a manlift basket that would not return to the ground. The Field PCS and the electrician on the ground attempted to bring the lift down on battery power it would not work. After verifying it is safe to jump the lift battery, the electrician was safely brought to the ground under the lifts power. It took about one hour.

A lightning strike caused one of our main Avista (Power Company) transformer to break down. We lost all power to the site and could not operate the Silver Shaft Hoist.

A hot spot on a coal pile was discovered at the Prairie Eagle East Portal Raw coal pad. No one was injured. Steps are being taken to transport the hot material to the prep plant to be washed and taken from property.

The door switch on the # 9 elevator quit working, a new switch was installed.

Hoist number 2 went down for an under voltage card issue. Hoist #1 was still out of commission waiting on an encoder to be replaced. At 5:00 P.M. we evacuated the mine due to two of our 3 conveyances being out of service leaving us only 3rd shaft. All miners were out of the mine and accounted for at 5:51 P.M. The hoist came back up at 6:40 P.M.

While dropping the hoist car into the mine, damage occurred to the hoist car axle that caused the car to de-rail. The hoist car was taken out of service while the slope track was examined for derailment issues and the brake car's axle was changed. No track issues were identified.

A mine fire was not extinguished within thirty minutes.

A NON injury roof fall occurred @ Cross Cut 17 in the #2 entry (Belt Line) on the Mudlick Mains. After investigation,it was determined that the cause of the fall was due to age deterioration of the area.

Encoder malfunctioned causing hoist to not function at 100 percent capacity. Hoist was downed until encoder can be replaced.

Teammate while holding the Manual override with their right hand inserted their left index finger into the Borax Valve to see if it was clean. The teammates stated their right hand slipped off the controls and the valve closed and amputated approx 1"" of EE's left index finger.

While swinging bolter head, employee tripped and fell forward and put right handout to catch their fall and left hand grabbed joystick which dropped bolter head onto their right hand causing 2 fingertip amputations.

A section of pipe was being removed from the plant dredge. The pontoons have a hook for a sheave that holds the electric cable. They had the cable removed and hung 2 sheaves on the pontoon hook. When hitting the pontoon w/ the workboat to separate the pipe and rubber elbow one of the sheaves flew off crushing employee's left thumb while they sat in the workboat.

IW was hooking up a small water trailer to a pickup truck. In the process EE fell and reached out to catch self. EE's left pointer finger tip was caught in between the trailer hitch and ball. Approximately 1/2"" of the tip was amputated. Taken to ER where they performed surgery to properly close the wound.

EE was closing the window on equipment. They caught their right pinky finger in the window, and amputated the tip of the pinky. Seen at ER, consulted with hand specialist at UofU, and referred to them for treatment. Seen on 5/10 and kept off work pending follow-up on 5/31. Pinky amputated to first distal knuckle. 50 lost days.

Worker was leaving at the end of the shift & saw a pin on an excavator track sticking out. They & another worker tried to reinstall pin with the forks of a skidsteer & the worker got a finger pinched between the fork & the track, causing partial amputation of finger.

Working on log wash and hand got caught in between log washer and the frame.

Worker was removing a rock from an airlock when the rock slipped and smashed their left middle finger between the rock and the housing. This caused a fracture and laceration to the employee's finger. Employee was placed on modified duty.

The workers were lining up some chutes, they were lifting them up with picking eyes, as they were hoisting the steel plate it got caught with other steel, when they were about to loosen the hoist the plate got loose from picking eye and dropped, nobody was underneath the steel plate but it bounced from another steel and it just caught one of our workers

Injured employee was checking engine fluids and fell.

The employee was sitting in the deck of a pinner moving it. The employee was holding to the top of the canopy with left hand, the canopy came into contact with the top mashing two fingers

A fully trained miner was installing a slider tube when they felt pain in their back. The injury was not the result of any compliance issues nor were any protective guards involved.

After installing new motor on roto packer and being down on knees for 45 min to complete installation and wiring, employee stood up and heard knee pop. Became reportable in September 2024 due to surgery.

Employee alleges that while drilling an angle to pass a screw, the continuous drill entangled the little finger of EE's right hand with the cable.

The employee stated : was putting up a piece of hog paneling when a rock fell out hitting employee in the nose

The employee was installing a roof bolt in the #5 entry, a piece of draw rock fell hitting EE's left ankle.

A fully trained miner while roof bolting stepped on a piece of coal and said knee popped. The injury was not the result of any compliance issues nor were any protective guards involved.

Miner fell from a ladder approx. 4 to 5 feet as EE was descending. Miner landed on a concrete floor. This resulted in muscle soreness and a concussion. Lost time began on 06-02-2023

Employee was putting grease on jack eye when jack slipped and pinched EE's finger in between jack and pin base.

Temporary staffed labor (C2605) was pumping diesel fuel when they used a knife to cut away an obstruction. The knife slipped cutting their left palm.

IE was raising up a high voltage cable over EE's head when EE felt a pain in EE's right shoulder. IE was taken off work for surgery on 9/25/2023. IE had surgery to repair a torn rotator cuff on EE's right shoulder.

Worker slipped due to the muddy conditions and hit forehead on the end of a steel bar as EE fell. Deep laceration required stitches at a clinic.

Employee was driving a motor carrying an empty trip. EE slowed down suddenly to check a switch and the trailing motor slid into the empty trip causing EE to hit EE's right shoulder inside the operators compartment.

A concrete slab was being placed onto a structure by crane, and the employee was using a shovel to help guide it into place. As the crane operator set down the slab onto the structure, the slab broke apart. A piece of the slab struck the employee on EE's upper back knocking EE to the ground.

EE was cranking a come-along that was attached to a floor panel. The floor panel broke in the Dryer and the come-along hook came back hitting EE in the mouth. Stitches, Broken Teeth and Fracture to bone in mouth.

Employee was getting back in the cab when felt their knee pop as they stepped down into the cab of Water Truck 24. The employee reported swelling and pain.

Employee was walking at x-cut 59 on the 12 section, when a piece of brow fell out striking employee on the left foot causing laceration to their foot resulting in sutures.

Employee was prepping to shear the fingers of a finger splice. EE started to shear the fingers with an oscillating tool and slipped with the shears causing the shears to strike right pinky finger.

A fully trained miner was going through a mandoor when the door shut on the miner's leg. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner injured finger when the miner dropped a rock on it. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was on a 6 foot step ladder getting ready to wedge the wall which was pyramided up in the middle. As employee was attempting to wedge the top of the wall, the wall started to fall toward them. The employee jumped from the ladder, landing on the pavement, injuring left ankle.

EE TRYING TO STRAIGHTEN BUMPER ON TR-11 USING CHAIN, 20TON HYDRAULIC JACK. EE USING ROSE BUD TRYING TO HEAT REAR BUMPER TO STRAIGHTEN. CHAIN UNDER TENSION BROKE, CHAIN HIT EE IN FACE/MOUTH. EE FELL BACKWARDS LOST CONSCIOUSNESS, STRUCK BACK OF HEAD.

Employee was pulling an old roller off of the beltline and felt a pain in lower back. EE continued to work until the doctor took EE off of work on 06/01/2023.

Employee states was pushing bolt to top when rotation hose got caught under motor at worm gear and pulled hose loose from fitting causing hose to strike operators hand resulting in 2 fractures to middle finger.

EE was ascending a ladder on a Haul Truck when EE felt a pop in their shoulder/arm. EE was evaluated by an Orthopedic, and was determined that EE tore the rotator cuff.

Miner carrying starter up stairs to change starter on crusher engine. Miner tripped on stairs with starter in EE's hands and starter fell on EE's thumb and smashed it.

As the Excavator operator started to access steps to enter cab, the operator felt dizzy. Operator stepped back to ground level and leaned on tracks to regain stability. At that time, the operator was taken to the hospital for observation which resulted in a Medical Complication (low Blood Pressure).

Employee was manually moving the position of tripper 3 with another employee when they felt a strain around the Achilles of their right foot.

Employee states they hit a pothole causing them to injury their back.

Miner was standing on a plank and plank slid.

Employee was using a water hose to clean at the 4 West #1 Belt Drive motor. When EE turned the water on, the pressure caused EE to lose balance and fall down. EE landed on the left knee, hitting it on the concrete pad/metal drive motor base. EE was wearing required PPE, has training and experience in job. No rules or regulations were violated.

Employee was helping get a mantrip through a track switch on the surface when the mantrip struck employee's right foot causing employee to fall and hurt left knee. On 6/28/2023 the operator received the report of an MRI showing a non-displaced fracture of the patella. This is now being reported as an NFDL incident.

At the face, Belting up. EE was pulling low structure over. While doing that, EE felt a pain in the stomach area. Later to find out EE got a Hernia.

Truck driver stepped down off the low boy trailer and stepped on to some chains that they had thrown off the trailer and twisted an ankle.

Employee was working on mantrip in the top shop. When kneeling and removing the u-joint bolts, the axle shifted and rolled off the forks, pinning the employee's arm to the floor. This resulted in injury to EE's right arm, wrist and hand.

Was measuring a hole to see if it was ready to be stemmed. The weight on the end of the tape got hung up. Employee pulled on the tape causing it fly out of the hole at a high velocity and strike the employee in the groin area.

An employee was sweeping the floor in the LO1432 Loader at the end of the shift. When EE straightened their back, EE noticed pain in the lower back.

While operating a 988K Wheeled Loader (Co # SL22), during normal working tasks, Employee stated was experiencing back pain with pain to the right leg. Employee indicated that the pain had become more intense throughout the shift.

Employee is an Equip Opr./Quarry Truck Driver. EE was operating a Quarry Truck, EE was waiting for EE's truck to be loaded when a rock ""spilled"" or ""fell"" from the Bucket of the WA-900 Komatsu Mach which was loading EE's truck, broke the driver's side window, hit EE on the L/thigh and fell to the floor of the truck EE was operating.

Employee was helping carry press belts to the belt line when employee on the opposite side dropped their side causing plates to hit employee's right knee. Employee started missing work on 5/30/2023.

IE was taking mono rail down when it spun on EE hitting EE's ankle causing a contusion.

Employee installed the outside bolt on the 4th row in the crosscut, while holding the steel EE dropped the pot to swing it in. As the pot dropped it came in contact with the steel EE was holding causing the 4 foot drill steel to penetrate EE's right boot resulting in a fracture to a toe and laceration to the foot

While tightening a ratchet strap the employee felt a pain in their right bicep. A torn bicep was confirmed and the employee was taken off from work.

Employee stated was lifting a 5 gallon can of hydraulic oil and the can slipped off car pulling employee's arm downward.

EMPLOYEE WAS DRIVING A JOHN DEERE TRACTOR HAULING A TRAILER WITH SUPER SACKS OF SALT. THEY STOPPED TO UNLOAD THE BAGS AND ROLLED AN ANKLE WHILE STEPPING OFF THE TRACTOR.

EE was cutting a bolt head off a plate that was secured in a vice. EE was using a portable circular saw. Saw blade contact the EE's left middle finger resulting in a laceration.

Lifting Screen Mesh onto screen deck with service truck. Screen hit conveyor and dropped hitting employee across the forehead. Hook on auto crane was in place and functioning.

Employee was changing brake pads on a haul truck. As the employee lifted the caliper up EE attempted to slide a jackstand under it. EE's hand slipped off of the caliper and it dropped catching right ring finger between the jackstand and caliper. Employee suffered a broken right ring finger.

An employee tripped while mounting a haul truck. EE felt pain in the shoulder as EE caught self. The employee was evaluated at the clinic and later (on 6/3/23) received medical treatment for the injury, making this reportable. The employee was not released to return to work.

An employee's left hand became lodged between the rotating pulley and the gear box shaft on radial stacker

While riding out of the mine on 5/21/23 employee stated was jarred in the seat resulting in back pain. EE did not report this to management. On 5/24/23 while on days off EE reported to the supervisor that EE was seeing a doctor for continued back pain resulting from the incident on 5/21/23. Restrictions were issued limiting EE from performing all required job duties.

Employee states the rotary production drill stopped suddenly while tramming to the next hole causing back injury.

Employee was tramming the left bolter from the inch tram control to install the last row of bolts when a piece of rock measuring 13 1/2' fell and struck the employee on the left arm and hip causing a bruise and scrape. EE went to Dr on 5-22 and Dr gave EE 2 days off work.

Employee stated was operating the 20-ton motor and had left hand on the wind shield and hit a kink and employee's hand slipped off wind shield and cut hand.

IE was carrying oxygen tanks to strap on the rib and a fellow employee dropped EE's tank on IE's left foot. IE was diagnosed with a bruised left foot.

A fully trained miner was standing in front of a scoop when the hanging bolter cable came down causing EE to hit EE's head on the scoop. The injury was not the result of any compliance issues nor were any protective guards involved.

(New Construction) While installing chute work, employee ascended extension ladder placing self in a pinch point area and the pathway of plate work chute being hoisted by use of chain hoist causing injury to left jaw

Employee strained left knee while installing a 54"" bottom belt roller.

WHILE EMPLOYEE WAS REINSTALLING THE LIDS ON A SCREW, THE BUELLS ABOVE LET GO AND DUMPED HOT DUST WHICH SPLASHED ON THE EMPLOYEE. THIS KNOCKED EE DOWN, AND EE CRAWLED OUT OF THE AREA. EE RECIEVED 3RD DEGREE BURNS ON SEVERAL PLACES ON EE'S BODY.

IE was walking down the slope when IE slipped. When IE slipped IE rolled an ankle. IE finished shift prior to requesting medical treatment.

IE was dragging a hose up the slope to wash. The hose caught and instead of going to release the hose the employee decided to try and jerk the hose free. When the employee jerked the hose it stretched and then it pulled back straining IE's shoulder.

IE hit EE's finger with a hammer when capping in a crib tie causing a fracture.

Employee was riding in a light vehicle coming out of the mine when a foreign body entered left eye. Employee washed it and saw site medic, was referred to an eye specialist, they were diagnosed with a rust ring starting and had it removed and was given restrictions for 3 days.

IE was building a kennedy wall when back started hurting. Dr. told IE to take some days off.

Employee stated was installing a bolt when a rock fell and smashed left hand on drill mast.

Employee was cleaning out the loader bucket with a scraper. EE jarred right shoulder, but did not seek any treatment until December 1. EE went to a clinic, and they suggested physical therapy, which helped. EE still had pain, so EE got an x-ray and MRI. It was then discovered EE a had a torn rotator cuff. EE is scheduled to see an ortho surgeon for further evaluation.

A fully trained miner was walking upstairs when the miner fell and injured ankle. The injury was not the result of any compliance issues nor were any protective guards involved.

The EE was carrying Cement blocks, when EE tripped over the plastic banding that secures the bundle of blocks. The EE caught self before falling to the ground and felt a pain in lower back. The EE was wearing PPE. Started missing work 5-18-23, Returned 5-23-23.

The EE was making a cable splice on the FCT, when the blade slipped causing EE to cut left knee with the utility knife. The EE was wearing all PPE. Began missing work 5-18-23, Returned 5-22-23

Employee was sitting in a parked mantrip on 2 East Track 57 wall when it was stuck by unattended slag cars. The Employee received 5 staples to the head and cracked spinous process C7 vertebrae. No other part 50.20-5 criteria apply.

Employee was walking through crosscut slipped on a small ledge and felt a pain in right knee.

Employee was stung by bees during outdoor housekeeping tasks.

Employee was in the work boat and working on the back of the dredge. The employee tried stepping on to the back of the dredge hitting EE's head on the exhaust pipe attached to the dredge motor. This results in the employee falling back into the boat striking EE's rear end on two guide bolts.

EE was attempting to remove a loose rock that was laying on top of EE's canopy when the rock slid off striking EE in the left shoulder. X-rays were negative. EE was taken off work starting on 5/17/2023 for a contusion to EE's shoulder.

Miner strained elbow picking up a drill fork.

While tightening a bolt on the crusher wear plates, the employees gloved hand slipped off the ratchet EE was using. EE fell back onto the other plates, fracturing EE's left femur.

GMS Employee was a passenger in a mantrip that was traveling in between a set of airlock doors when the mantrip was accidentally struck by slag cars causing injury to the employee.

Injured employee was bolting off of the miner. While pushing a roof bolt up into the bolt hole, employee felt pain in EE's right shoulder. No rules or regulations were violated and employee has training.

While checking a splice on EE's shuttle car cable, a piece of rock fell from above the coal seam that was on the rib/mine roof and struck the employee's left leg causing a fracture to EE's tibia. EE began missing work on 5/17/2023.

Employee was sitting in a parked mantrip on 2 East Track 57 wall when it was struck by unattended slag cars. The Employee received 3 staples to the scalp. No other part 50.20-5 criteria apply.

A fully trained miner was installing a bolt when a roof strap cut their hand. The injury was not the result of any compliance issues nor were any protective guards involved.

After an employee exited a Kawasaki side by side, EE rotated on EE's left foot to reach EE's lunch box in the back. When EE turned back, EE felt a pop in EE's left calf muscle.

The EE was operating the miner on the 5W section, and was retrieving a bolt from the bolt tray located on the miner when a piece of slate/rock fell from the mine roof striking the EE right index finger. The EE was wearing gloves at the time of the incident.

Employee was standing on the discharge side of the pump while making sure it was primed when the discharge hose came apart striking EE in the right leg causing a bruise. This resulted in a lost time injury.

Employee's foot was pinched between the bolter pot and ATRS mounting bracket of a Joy 14-09 continuous mining machine.

Employee was loading a customer truck from T243 and turned to close a gate when employee felt a sudden sharp pop in lower left leg.

While cleaning up P77 drift the mucker bucket hit the rib, causing an abrupt stop - forcing the operator's face into the windshield. The operator received several laceration on EE's face.

A gravel truck hauling sandstone from the mine site ran into the ditch line while attempting to pass an empty coal truck coming on to the mine site. The accident resulted in deep bruising to the Mar-kel truck driver and lost work days.

IP was using chain hoist to raise sump pump when the hoist stopped. IP jerked on the chain and felt a pain in shoulder. IP continued to work the remainder of the shift and the following day. Injury was reported to operator on 5/17/23 and accepted by WC on 5/30/23.

A fully trained miner was carrying mono-rail when they stepped in a hole and the mono-rail hit their right shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

The EE was in the process of securing a belt scrapper with a yellow rope hanger. The hanger caught something on the EE pulling their right arm into the belt roller. The EE was wearing the appropriate PPE. The EE began missing on 5/16/23.

Employee stated that they injured their right elbow while reaching to mount a ladder. Employee was taken to the clinic for evaluation and returned to work. Upon a follow-up appointment, employee was referred for an MRI. On 5/31/2023, MRI results indicated a torn triceps tendon at right elbow. Employee was referred for surgery, has not yet been scheduled.

5/12 tripped and fell on an uneven sirive screen plant (S190) 5/25 slipped getting off a loader (Cat 981G loader co#1071)

Employee was walking the NE6 belt when employee tripped on a rock causing employee to fall, coming in contact with the belt resulting in abrasion to right shoulder and elbow. Employee has continued to work since date of injury until 6/28/23 when employee went off work for medical procedure.

Employee was pushing off muck bucket and tripped. Bucket came back and pinched ankle between bucket and mucker causing pinch/mash/laceration to right foot.

employee was stepping on to the ground from the last step of a haul truck hydraulic ladder at the end of EE's shift. EE states EE felt pain in EE's left hip. The ladder retracted close to the ground leaving the last step shorter than the rest

While using a sledge to remove a hammer a piece of metal broke off and entered left leg.

The employee reports today, june 16, 2023, that on Monday, June 12, 2023, while working on the installation of a water pipe on the Cantera property there was a shot to extract material and that with the wind the dust reached the area EE was working and fell into EE's throat. EE indicates that at that time of the incident was not wearing mask.

The Employee was walking into work (around 6 am). EE was carrying lunch box. EE stepped off the edge of the sidewalk. This caused EE to roll an ankle (no injury) and fall onto left side (shoulder was dislocated). EE fell just past the entrance to Haile's admin building (near the turnstiles).

The employee was mixing Nitrate and Diesel fuel in the Bulk Truck to produce Ammonia Nitrate. As the Nitrate slid down the bin, the flow of Nitrate stopped because of a crust build up. The employee used a digging bar to break the crust to resume flow. When the bar broke through the crust, the employee fell forward striking the lip of the bin.

Employee was walking in the No.4 to No.5 crosscut and tripped over the C/M cable and fell injuring left and right knees.

Employee placed foot inside pallet while stacking bags. Coworker lowered the pallet lift, catching the employee's foot. Employee bruised foot and was prescribed ice and NSAID's. Resulted in LTA for facility. Employee returned to work on 5/16/23 with full medical clearance.

Employee was collecting blast hole samples in the East Pit on 5/11/23. The employee reported pain in their lower back during sample collection. On 5/16/23, the employee notified their direct manager that they were not reporting to shift due to fatigue, which they attributed to back pain.

Employee was installing ventilation curtain and tripped and fell injuring right knee in the process.

Mechanic was removing a head pulley from conveyor pushing it by hand off the end of frame work and into loader bucket. Mechanic's hand got caught between loader bucket and head pulley resulting in multiple broke bones in left hand. Loader was stationary the entire time.

The miner was using the skid steer to load up trash and debris left on shore from the dredge to take to the dumpster. Miner had feet propped up on the skid steer steps, and simultaneously lowered the bucket which came down on miner's foot. Minor injuries (foot contusion) was sustained.

Employee was rib bolting off the miner at approximately 24+40 survey station. Upper portion of the rib became dislodged striking the legs of the employee, as a result they fell hitting their head on the miner. The rock measured 58"" long x 30"" high x 6"" thick. EE suffered a hematoma and concussion like symptoms. No compliance issues were found. No other part 50 criteria apply.

The miner was removing a section of an elevated walkway when the walkway beneath EE fell. The miner fell approximately 18 feet to the concrete pad below. The miner received injuries to the ribs, elbow, kidney and spleen. The miner is now nearing full recovery from injuries, but has not yet returned to work.

Setting up the plant, operator of end loader had placed the conveyor hopper on blocks. They backed up and the chain hook caught the conveyor/hopper and dropped on injured employee's foot. EE had steeled toe boots on and got no broken bones, but the toes were pretty squashed. EE had stitches and developed compartment syndrome in their calf.

Pulling a bearing off feeder shaft because bearing was down while pulling it off chain, slipped off bearing, striking them on the right wrist.

Injured was replacing rear end of a scoop on the 5W miner section. When EE went to stand up from being prone EE felt a pop and pain in lower back. No problems with equipment or compliance issues, training, or PPE.

Employee was dismounting a rock truck. Employee stated they looked over at another piece of equipment for their lunch box and missed a step losing balance. Resulted in the employee falling to the ground and injuring right ankle/shin.

Drill steel came out of the carriage and fell in to the housing. We then laid the tower down to maneuver drill steel back in to place. While doing this the drill steel kicked back knocking EE off the drill rig on to the ground. EE sustained a nose bleed and back pain. We then called an ambulance and EE was transported to a hospital.

Injured was an employee riding in a track mounted man-trip, when it derailed and struck the coal rib causing the man-trip to stop suddenly. The employee was thrown into the man-trip, and EE suffered a laceration that required one stitch and blunt trauma injury to EE's right eye. EE was wearing safety glasses at the time.

A fully trained miner was bolting when a rock fell from between the straps and hit them in the back. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee states they hurt their back while lifting a plate door off a transfer chute access.

Employee was getting off the company provided bus in the parking lot, upon stepping down on the asphalt employee's left ankle rolled, resulting in a sprain prior to starting shift. Seen site medics, referred to ER, x-rays were negative but restrictions were issued for the swelling.

EE was moving bunking for a saw block and stepped on a small stone and twisted an ankle

Our employee, was washing off crusher deck when a bolt fell out of the crusher box striking them on the left shoulder and their neck.

Employee was moving a rock to set a timber when EE felt a pain in EE's right shoulder. Employee initially sought medical treatment and was released to return to work. Employee went to a follow up appointment on 09/26/23 where EE was advised that the MRI that was given on 08/16/23 showed a labral tear of the right shoulder that will require surgical repair in the future.

While Dismounting the tire handler and progressing down the ladder, EE stepped down to the last rung on the ladder, felt pain in right ankle and could not put any weight on right ankle/foot.

The employee was manually loading a wooden pallet into the robot pallet feeder, the employee felt pain in their left shoulder. This task occurred numerous times previously in the shift. The employee initially took Ibuprofen for the pain. The following day, the employee went to urgent care and was removed from work. X-rays showed no fractures.

The EE was attempting to replace the fuel filter for the excavator, when diesel fuel splashed on the employee.

While pulling rope slack back from a motor their glove got caught in the rope and pulled their hand partially into a sheeve wheel causing a contusion and abrasion to their left ring finger. No problems with equipment or compliance issues, training or PPE.

The employee injured left thumb while helping to secure a V-belt guard. As employee was holding pressure using a wrench another employee used an impact wrench to tighten the bolt of the guard. Not being aware of surroundings employee placed left thumb between the guard bracket and structure framework.

While cleaning under the conveyor belt, the employee's arm and shoulder contacted the conveyor frame, causing a skin tear on the left elbow and acute pain in left shoulder.

Employee was moving bed rail to the rib when right middle finger was caught between the bed rail and the rib. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

at the end of the shift the employee was leaving the plant when they slipped in the mud and fell, hurting their left arm

A shuttle car operator got stuck. A battery scoop operator pulled it out, the car rolled back down, they started the car and backed up to give chain slack. They bumped the scoop operator with the car on the upper shins. The Maintenance manager took them to the doctor. the X Rays taken revealed that there was a small hairline fracture in the upper right shin.

Employee was lining up a jack pad on undercutter #2. The steel slipped when attempting to line up the jack pad and pinched EE's middle-left finger between steel frame and pad.

Process operator was connecting water line to Hydrochloric Acid pump to flush pump for scheduled repairs. The PVC fitting broke and the operator was sprayed in the shoulder and face.

Employee was setting a Heitzman jack when a piece of roof rock fell measuring 3 feet x 2 feet x 8 inches thick. The rock hit employee's right foot.

Employee was attempting to reinstall a rub rail on the continuous miner when it fell, striking employee on the top of right foot. Employee sought medical attention and was diagnosed with a transverse fracture of the right metatarsal bone in the right foot. Employee was given a full release and returned to work on next regular scheduled shift (5/8/2023).

While checking the torch on K1, employee noticed an air lance inserted into the cyclone on K2 hood and when walking to check why it was there the material blew out of the pipe and onto their arm and face.

Employee was attempting to cross the belt line and had one leg crossed over and they under estimated the distance to the mine floor while the other leg was still on the belt and their leg impacted the metal belt rail with all of their weight on that leg as their leg slid down the rail and this resulted in a torn muscle on the inside of the leg.

Employee had a personal medical condition causing loss consciousness and falling to the ground and fracturing orbital bone.

Employee was bolting in the #5 entry, when removing the steel from the roof the finisher separated from the pusher allowing the pusher to come out of the chuck striking EE in the safety glasses on left side of the face, EE was taken off work at this time

When loading a roll of conspec wire into the back of a vehicle, shoulder was strained and roll dropped into ride.

A fully trained miner was shoveling belt when they felt a pop in their back. The injury was not the result of any compliance issues nor were any protective guards involved.

Miner was setting up a temporary work platform when they slipped and fell to the conveyor below them.

Employee was riding on a diesel mantrip into the shop, at the end of the shift. The tip of EE's left boot was sticking out and it got caught between a small post on the mantrip and a ground-mounted metal post that was mounted in the shop "" garage door "" entrance. EE received stitches on EE's large toe.

Employee was leaving fuel dock attendant shack when gust of wind caught the door resulting in employee twisting their knee.

EE was plugging charger cable into a battery scoop. EE felt a pop and burning sensation in right elbow.

Employee had filled tank on truck and was taking fuel hose out of tank and as EE stepped down from truck EE slipped and twisted left ankle.

Employee was exiting EE's shuttle car when EE stepped on coal and twisted EE's ankle. An MET applied a cold pack and an Ace Bandage. EE went to the doctor the following day and was given a full release to return to work.

Driving a forklift underground on hit a few bumps and got a sore back and no pain that day. on 5-7-23 their back started hurting so they got it checked out at the ER. The doctor let them finish the shift and see a ortho if not better in 2 weeks. on 5-31-23 they got results and got an no work release.

IE stood on a belt conveyor to inspect a stone feeder that may have been leaking. IE did not lock out the belt prior to accessing it. Conveyor belt started up and pulled the IE through a 14' height clearance before IE was able to pull the E-stop cable and climb off the belt. Incident caused multiple fractures in employee's back requiring EE to have surgery.

Employee stated was lifting a valve approximately 40 lbs and strained back.

Employee twisted to pick up a 5 gallon oil can to put oil in the cutter and says that pulled something in their back when EE twisted.

Employee was inspecting tracking on a bagging belt. When they squatted down to inspect the belt from beneath they brought a hand up and the belt caught their glove pulling their thumb between the belt and a pulley.

The employee was climbing out of the deck of a scoop. They put their left foot on the ground and felt pain in left groin.

The EE was walking around the curtain of #6 seal of the 3 North Rehab, when a rock fell from the mine roof and struck the EE's left shoulder. The injured finished the shift, and began missing work 5/3/23.

A roof bolter's right uniform sleeve was caught by a roof bolt plate as EE was installing a roof bolt in the #8 heading. Both forearm bones were broken and EE received a cut to the right ear that required stitches.

Employee complained of ""lower back pain"" after exiting the loader for a mid morning break.

The employee was manually stacking 75LBS cement bags. After doing so, the employee recognized pain in lower back and leg. On May 15,2023 the employee completed the accident notification to the company. After a medical evaluation, the employee was returned to work with a restriction and having been prescribed medication.

Employee was helping lift a spool of leaky feeder cable onto a 4 wheeler and felt a pain in their back.

Ee tripped on welding screen and fell on elbow. Diagnosed with a fractured olecranon process and referred to orthopedic surgery.

Employee was installing a chute on the back of a concrete truck and the chute came down on EE's middle finger cutting the tip of the finger.

While loading a top roller into a scoop insert, EE stated felt a pop in lower back.

The EE was improperly loading steel into a drill. They laid the boom down & were adding steel directly into the carousel. One end of a steel fell out while handling it. This caused the other end to fall out & smash their thumb between another steel. Upon seeing a doctor, it was diagnosed the the EE ruptured a tendon in their right thumb.

EE was pushing the drill steel to the roof. As EE let go of the drill steel when EE was starting the hole, the joy stick stuck in rotation and up pressure. The drill steel bent and came out of the hole spinning and struck EE in the right hand 3 times before EE could get away from it.

Employee states EE was working on the ball mill trying to fix an oil leak. EE went to leave and lost EE's balance and grabbed the handrail, EE then felt burning and sharp pain in EE's left shoulder area.

Worker was splitting stone slab with hammer and wedge. Metal wedge broke off and a small sliver pierced EE's Left hand.

EE has ground level fall while exiting from Bdg#2. EE stepped off traction grating and onto slick pavement from rain event prior, causing EE to fall forward and landing on right hand and knee. EE diagnosis: acute right knee sprain

Standing on the adjustment springs for the lower curtain liner, EE went to step aside to adjust self when EE's foot slipped on the adjustment screw and fell to the deck of the inertia, landing on left leg and at that point went down onto the deck of the inertia holding knee.

When exiting bathroom, associate stepped on a rock and rolled right ankle. The employee saw an ortho on 6/7/2023 and was signed out of work when an x-ray revealed a broken bone in EE's leg.

Miner was trying to move a piece of granite away from splitter to gain access with a lift chain when EE's finger was crushed between two pieces of granite.

IP was driving a haul truck and began developing lower back pain. Upon arriving at the surface shop EE's back began to hurt and EE was unable to pick up the wheel chocks.

EE has fallen attempting to climb the CR3 tiedown berm (the height of the berm is approximately 3FT). EE had dismounted 329 haul truck with EE's backpack on and water jug in hand and was completing EE's post op inspection when EE fell climbing the CR3 Tiedown berm.

An employee was injured when a hammer fell through the collar doors as they opened. The employee was on the work deck cleaning up after a concrete pour. The hammer struck the employee on the right shoulder causing a laceration and fracture.

The employee was installing new buckets on FM#3 discharge elevator. The bucket weighted roughly 25lbs and was lifting the bucket at chest level. Employee has been doing physical therapy from 5/25/2023 until 1/31/24. The doctors diagnosed EE with a torn rotator cuff and went into surgery 1/31/24.

Speaking with co-employee running skid steer. Turned around to move stone stacker and tripped over skid steer attachment. Nondiscplaced fractures of head of right and left radius. Restricted days started on 5/26/2023.

Employee was using a cutting torch to cut I-beams at the metal scrap yard, when they stepped back to let the cut piece drop, they tripped and fell on to an I-beam hitting their tail bone against it.

Felt a pop in knee while exiting rear of straight truck

EE was mounting a haul truck wheel. Stood tire and wheel up to sit the lock ring, turned to walk away to continue working, and felt the rim fall on them. Initial report is that the rim ""popped"" out. Investigation is ongoing. EE transported to hospital, diagnosed with multiple fractures, and transferred for surgery. Surgery took place approximately 10 am on 5/24.

Employee advised Manager that they bumped their right knee because they could not move the seat. Manager got in the haul truck and the seat did move back. EE reported the knee was fine, then on Saturday the 27th sent a text stating they wanted to go to the doctor on Tuesday, May 30th. They had just been released to full duty 5/15/23 from a left knee injury.

Employee reports injury while descending steps on the M/V White Rock. As the deckhand was going down the Engine Room steps, employee slipped on the second step from the floor and attempting to stop the fall injured right shoulder. Employee also suffered a small avulsion on right index finger.

Employee's were installing a new auger in a wash plant trying to align bolt holes in the two flange assembly. The employee put the tip of EE's finger in the bolt hole to align up the two holes. One of the two flange pieces slid down and lacerated the tip of EE's right middle finger.

Employee riding in mantrip vehicle when it hit a bump causing the employee to be jarred, resulting in lower back pain on left side of back. Seen by doctor and given a full work release. On 5/25/23 employee was seen again due to persistent pain at which time, restrictions were issued limiting the ability to perform all required job duties.

Employee was walking around Drill 9 when they tripped and fell and hurt the right ankle and scratched the right hand. The employee was evaluated by Site Rescue. The incident resulted in a right ankle fracture.

An operator was inside of a drill & noticed the door's rubber seal was coming loose. Operator opened the door to adjust the seal with right hand & then closed the door with left hand. Operator's two fingers on right hand became smashed in the door frame. Operator was taken to a medical facility & sustained a fracture to a finger.

Employee was trying to loosen a bolt on a continuous miner lid when they felt a pop and pain in left bicep. Employee worked until scheduled surgery on June 13, 2023.

Employee was removing rock on top of conveyor belt when belt rolled and pinched EE's foot in hopper. EE had a fracture to EE's left fibula and was placed into a splint.

IP was being trained in the use of a hose making machine when the trainer was demonstrating how to place dies. While in the process of closing down on the dies, an object struck the IP in the right shoulder.

Miner states EE was jogging from the VSI to the control house stairs when EE twisted EE's ankle. Injury sustained was diagnosed as an ankle sprain.

Employee was climbing up onto the back of utility truck. Said employee lifted their left leg up onto bed of the truck then used both arms pull the rest way up. Employee said they felt pain in left shoulder.

Stepping down from ladder on crane when took last step down stepped on rock on ground injuring right knee.

Employee was inspecting the K1 front clinker tunnel to ensure that the other shifts have been doing there assigned tasks. During inspection they found that both the north and south side of the tunnels had spilled material so they went ahead and started removing the build up by shoveling when they encountered a sharp stabbing pain in lower abdomen right around the belly button.

Employee was shotcreting in an entry & got hit in the right eye with a piece of material.

Employee was working in a low clearance area bent over and when employee turned to right knee seemed to collapse and employee went down on the shop floor. Employee stated that the right knee was not working, and employee could not get up.

Employee was walking on a slight incline when they twisted their ankle.

Employee was attempting to use an overhead crane to straighten a piece of metal on the bed of an overburden truck. When tension was applied to the crane hook it moved and pinched the employees thumb resulting in an injury.

Employee arm was sore after opening valve on tailings line.

Haul truck driver closed the truck door on their own finger.

The IW had put degrit screens 2-4 on water and was doing routine checks of the valves and walked into a low clearance valve handle hitting EE's head. IW was wearing a hard hat and injured EE's neck.

Employee was descaling 2/3 Hopper when a piece of scale fell and hit the gun. The gun in turn struck the employee in the face area

Employee was clearing debris from dump truck tailgate and pinched finger in the tailgate. Received cut to middle finger requiring stitches.

The employee was removing a rotex screen from the screen rack on the 3rd floor and felt discomfort/pain in the left shoulder.

Employee supplying packaging line with empty cases when felt discomfort in lower abdomen right side. On 5/15/2023 employee saw physician and diagnosed with inguinal hernia. Placed on restrictive duty.

The miner was climbing down from a scaler and missed the last step falling backward onto a nearby pickup truck hitting EE's back.

Employee was lifting rubber mill liner segments off forklift & placing on liner machine to install inside the mill. Employee felt EE's forearm pop. Employee notified supervisor & was taken to ER. Employee was released to return to work w/further exams scheduled later. MRI results showed torn bicep requiring surgery. Employee missed first scheduled shift on 6/1/23

Employee rolled their ankle while stepping from stairs to a walkway.

Employee was doing a maintenance repair on the cone crusher and could not reach the hose that needed repair. The Employee grabbed a 5 gallon pail that was close by to reach it. While standing on the pail the employee slipped and twisted knee causing left knee to be sprained.

Maintenance employee tore their bicep while lifting a tool bag with a rope.

EE was performing dye test on dust collector when door to compartment slammed shut on right hand. EE received several lacerations to hand.

Employee was retrieving a hose off the rib, lost their footing, twisting in the process, injuring their knee. Note: Employee was released back to work after being seen offsite. After their next rotation the employee saw an orthopedic specialist and needed surgery. Employees first missed day after surgery was 07/27.

Mechanic was using a spline air gun when the bolt broke cause the employee wrist to twist.

Employee's foot sunk into material inside the SAG mill while pushing bolts through the bolt holes for a liner change. This action caused the employee to twist knee suffering a strain.

Employee shoveling sand. Stated that they felt a pop in back, felt the pain, got light headed and fainted. There were no witnesses. EE got up on their own and walked to Operations Manager and to breakroom. Taken for medical care, was diagnosed with Thoracic Spine Strain. Issued a prescription and not allowed to return to work until 05/09. Restrictions-no heavy lifting 5 days.

Employee was performing EE's pre-shift inspection on Loader 944K2 and went into the lube shed to get a few rags to clean the windows on the loader. As EE exited the building and was walking back to EE's loader, EE struck EE's right shin on a large set of wheel chocks that were stacked together in front of the building.

Employee walking through a doorway carrying two buckets of tools. Tripped and fell on shoulder. Initially got better then pain did not go away. Went to doctor months later and found tear requiring surgery.

Employee was getting out of the cab of the JCB and missed the step. EE didn't grab the hand rail first and then fell backwards landing on EE's lower back and butt.

Supervisor was using a hammer to remove bearings for the wash plant shaker screen, when the hammer struck a bearing directly. The bearing shattered and some of the splintered pieces struck EE as EE was standing approximately 12' behind the supervisor. One piece struck EE's chin and another embedded in right forearm.

Leach Employee picked up portable generator and strained back.

During installation of brace pin on shovel dipper, employee hand was pinched between pin and equipment being used in the installation.

The employee experienced back pain while performing preventative maintenance on a conveyor belt.

Employee was manually changing slurry filter and turned to set it on the floor when EE strained lower back.

Employee was removing lug nugs on stockpile truck tire using breaker bar with etension. Lug nut broke loose suddenly, causing sudden shock to wrist area, resulting in swelling and pain in Left Wrist.

The employee was lifting the tool bag out of the locker when employee felt a pain in left groin area. The employee had completed morning stretches prior to this event. Employee was seen the next day at the clinic where a 5-10 lbs lifting restriction was placed on employee to aid in the healing process.

Employee hit finger with sledge hammer while driving pin into place

Employee struck in face by a Shive Puller while trying to remove shive from a Raw Coal Screen on 4th floor plant 2B.

IP was directing vehicle traffic and tripped on a concrete ledge resulting in a sprained ankle.

Employee was pulling on the v-belts to spin the dust collector fan. Once EE stopped pulling on the belts, the fan rolled backwards pinching EE's fingers between the belts and pulley.

Employee worked on multiple jobs this day. One of them involved replacing a gearbox on a screw conveyor. Employee noticed EE's back was sore after carrying the gearbox up the screw conveyor.

Employee was working at the densifier and was moving pallets onto the densifier deck when EE felt a sharp pain in the shoulder. Employee then helped re-rack a bag strap and the pain presented itself again.

While operating an excavator, A rock fell off the truck being loaded and struck the employee's lower right arm. This resulted in the employee having 2 stitches.

Unexpectedly, the material fell from the west side of the TA duct, splashed on the ground and spread widely, creating a dust cloud covering the barricaded area and reaching the worker placing a manlift outside the barricaded area. The employee suffered from corneal abrasion and received prescribed medication and work restrictions.

EE was walking out of loading tunnel onto uneven grading and felt pop in left ankle.

Employee was operating a Sky Trak forklift and rolled backwards. The left rear wheel went up on a berm causing the forklift to lay on its side.

Employee tripped over a parking block and landed awkwardly on EE's right leg straining EE's hamstring.

Employee was feeling underneath engine for hose and cut their right thumb on the fan shroud.

Injured employee was opening the access door to a Rotex (mineral separator) when they pinched the palm side of their right ring finger at the first knuckle between the door handle and latch, causing a laceration that required three sutures.

While operating 40t CAT haul truck, employee states steering failed causing haul truck to strike clinker cooler building wall. Employee complained of neck and shoulder pain and diagnosed with sprains of the neck and shoulder. Employee was placed on restricted duty.

Loader was loading a bulk truck. While handling a vacuum hose, the loader experienced a pull and pain in right shoulder.

Employee was exiting a motor grader, EE was facing the steps of the grader while holding onto both handles. As EE was completing the dismount to the ground, EE's right foot slipped on uneven surface, and EE twisted right ankle which caused EE to fall.

Miner was using a razor knife to cut belting material. Hand slipped and cut left index finger, requiring 11 stitches.

Employee stated while using a battery operated grinder to cut a bolt off of a belt roller bracket, the grinder recoiled back striking EE's left knee, resulting in a small laceration requiring 6 sutures

EE was raking under the idler 7B conveyor when the rake became caught in the belt and broke. The employee's hand struck the support beam and the contact caused the employee to sustain several lacerations to the back of their hand. EE was issued restrictions.

The 417HT was being loaded by the 24S at the 5950 bench of Little Rock. While swinging the bed of the truck the dipper made contact rubbing against the bed of the haul truck. Employee was taken in for observation to the ER and was placed on 1 day of restrictions.

EE was unloading cement at the rear of EE's trailer, the loader operator on site was filling the pug mill screen and sent rocks off the back side where EE was standing. EE ran to avoid being struck by rock tripping over a discarded wheel chock rolling EE's foot and fracturing EE's big toe.

IP had cut a sheet of mesh down to finish bolting to grade line. After cutting the sheet they moved the wire into place, while moving it one of the cut ends of the wire hit them in the ankle. The piece of wire penetrated the IP's boot and socks and punctured into the skin near their ankle.

IP was repairing wireline cable, when IP pulled on wireline cable, the cable slipped through their hand, causing the cut end of the cable to slice through their nitrile exam gloves lacerating their left palm just below the thumb. IP was given 7 stitches by the site medic

While washing the pad under the crusher, the employee did not realize that they were close to the edge and slipped off. The edge of the pad is approximately 8 inches tall. When the employee fell they landed on their right elbow causing a dislocation.

Injured employee used a bar hook with a T handle to snare the cable of an air hoist to pull it towards themself to retrieve the ball. The employee's left index finger was caught in a pinch point between the bar hook T handle and the rosette of a vertical scaffold leg. This resulted in a pinch laceration requiring stitches

As the employee lifted the drill steel with the drills jib the steel got caught on the drill head, causing the bottom of the drill steel to kick towards the drill rig. When the drill steel kicked towards the drill the employee put EE's arms out to redirect the drill steel into place and from hitting the drill. The pipe hit EE's forearm against the metal frame of the safety gate.

Incident occurred while employee was using rotary hammer from man-basket to remove build up on crane way wall and pipes. Employee was hammering build up on pipes when it became lodged in build up. Employee let off the trigger was a few seconds then engaged trigger to try and pull out hammer. While pulling on hammer employee felt pull/ strain in middle/ center of back.

Miner was negotiating stairs to perform maintenance on a screen. When they turned to face screen, their foot slipped off of the stair tread causing them to trip down the remaining two steps and sit down on top of their right foot/ankle. Miner was evaluated for right ankle strain.

Injury was reported to employer on June 20, 2023 at 12:30 PM. Employee states, in mid-May IE was helping another EE change the top screen cloths on an 8x20' middle deck. The other EE cut the screen in 1/2 while IE was pulling up on the half screen. While pulling up IE states IE's neck and spine began to hurt.

EE was injured after exiting a Skid Steer. They jumped out of the cab instead of using 3 pints of contact. When they landed on the mine floor, they twisted their right ankle. X-rays were negative. Diagnosis is sprain/strain to the Right Ankle. EE's ankle was wrapped and they were put on crutches and given modified duty until follow-up appointment.

EE was working near conveyor belt 10 as the vacuum truck operator. The employee was walking back to the vac truck and tripped on a rock causing EE to fall and land on EE's shoulder. EE dislocate EE's right shoulder

Worker went to doctor with reaggravation from previous injury on 2/22/2023. Worker sought medical treatment on 5/16/2023 when off and contacted Manager with a note for restricted duties. Worker had previously been cleared for full duties from the 2/22/2023 injury.

EE developed back pain while on shift. Initial cause was not determined. EE submitted an additional report three weeks later stating the back was hurting from shoveling material and sought medical treatment and received work restrictions.

Ee was lifting boxes of crucibles from a pallet to stacks for weigh-up. Following this task, they reported feeling pain in low back. Seen at clinic, xrays negative for fracture, diagnosed with lumbar strain and sacral/low back pain. Released with restrictions that site can accommodate.

EE was using a hoist to lift the copper bags at #2 mixer. The injured EE tried to reposition the chain using the lifting straps when the hoist stop lifting. When the chain cleared the hoist housing and proceeded to lift the straps tightened up on the EE pinky finger.

Ee was moving a barrel and it shifted, landing on left foot. Ee was seen at clinic where xrays were negative for fracture. Diagnosed with left ankle strain and released with restrictions site can accommodate.

In the process of changing hammers, EE was unscrewing it and lost control of the hammer injuring left index finger. (Cut & bruised.)

IW was moving a pallet from a stack to the floor, dropping it on the ground. Pallet tipped forward striking the employee's wrist causing injury

Employee was hit on the hard hat when the haul truck tire ring slipped and fell.

While shoveling by the mainline 2, the miner noticed a pain on EE's right side, 3 inches below EE's armpit.

Hit left hand with hammer, while trying to move pin from from pulley. Broke left thumb.

Employee was walking into the impoundment pond from the trench when they missed a step and fell back to the ground. They lacerated their head with the plastic bracket in the hard hat. EE was given first aid and continued to return to work. EE sought medical attention the following day.

Employee was working in a quench pit and got boot stuck in 2"" of mud. When the employee tried to free the foot the employee twisted / strained knee.

Jaw crusher clogged. Rock bound in jaw. Lock out the feeder. Manually dropping rocks to break it free. Crushed finger btwn rock and jaw wall.

On 5/12/2023 at approximately 4:40pm EE was removing a trough roller from under a crusher plant. While EE was pulling the roller EE mashed right thumb between the roller frame and the frame of the crusher. EE was treated at the ER and received 2 stitches and a fracture to a bone in thumb. EE was put on restricted duty.

On 12MAY23 EE slipped while using a pressure washer. EE grabbed a nearby support beam to prevent falling. EE reported incident on 15MAY23. Requested medical attention on 16MAY23. Diagnosed with right rotator cuff strain.

Transport van passenger hurt their neck when the operator stopped suddenly for a pot hole on the haul road.

Employee was driving a haul truck after dumping on middle A dump. As the employee was driving down the ramp the truck hit a bump jarring the employee's wrist. The employee stated that it didn't hurt at first but started to swell up over lunch break. The employee became restricted effective 5/12/23.

Employee states the steering wheel of the service truck being operated jerked after hitting pothole hurting their shoulder.

They were adding a pipe to the sand slurry line and the flanges weren't lining up so EE pushed it.

Employee was changing a tail pulley bearing. The all thread part of the take up was ceased up. Once the all thread broke free, while using a break away bar, the employee's left middle finger was mashed between a lifting eye and break away tool. Resulted in a laceration and broken finger tip.

Employee was off loading pipe from flat bed trailer when pipes band broke, causing it to break free of its restraining devices and roll off trailer, striking employee's right shoulder.

Miner was greasing belt take up/ bearings and roller while belt was running to identify which bearing was squealing and going bad. Miner removed guard to conduct greasing and while greasing the grease line got caught on belt and pulled miner's hand under roller between belt

Employee was using a 1/2"" drive torque wrench and while in the process of torqueing bolts and felt discomfort in the right elbow. Employee was given a sling at the ER visit.

Employee's foot slipped while dismounting water truck straining their shoulder.

EE was walking down some stairs and EE's hand slipped off the handrail. This caused EE to stumble and loose EE's balance. When trying to recover from the slip EE stepped on a curb and rolled EE's ankle.

Haul truck operator hit pot hole causing lower back pain.

Employee reported the bed of EE's 777 truck slammed while retracting after dumping the load at the main crusher. EE did not feel any pain until April 28th, 2023. The initial report of injury did not require any restrictions, however, on EE's second visit, after an MRI EE received restrictions on 05/08/2023

Employee stepped down approximately 10"" onto grating, stepped on wash-down hose causing their ankle to roll.

Employee was walking up the staircase of the Screening Tower when they did not gain proper footing on the final step, fell and hit their right knee causing discomfort.

Employee was attempting to open tk 67DOV with a sledgehammer when they felt discomfort in their wrist

While drilling out a cable fitting the drill bit pierced the employees glove and the outer portion of their left thumb. The employee received stitches.

Employee was changing a drill bit and struck left hand.

Contractor miner was tasked with putting the guards that go over a feed belt back on. After leaning over to tighten the guard EE relaxed their weight on the handrail and the horizontal arm on the handrail gave. The contractor then lost their balance and their torso fell on toe plate. The toe plate was 45 degrees outward when the accident occurred

Employee was cleaning up cement that was on the floor under the cement line palletizer and was bent over using a come along to pull out the cement that was under the cement line palletizer when EE stood and turned and fell and twisted EE's knee on EE's left leg.

EE stepped down to the ground from cable boat located in shovel cut, G5 North. EE's left foot contacted the level ground and rolled their ankle.

Service technician working on engine removal from a Skid Steer Loader and with impact gun on a fastener lacerated right middle finger while other hand was exposed to the other end of retainer.

EE was working in removing petron barrels. EE was helping a skidsteer operator by holding & guiding the barrel into the bucket of the skidsteer. The barrel started to tilt over & to prevent it from falling the skidsteer operator put the bucket down, pinching the EE's foot. EE was take to clinic & issued restrictions.

Was stacking rock, my feet slid out from under me, the rock crushed my hand.

Employee experienced discomfort in their right wrist while shoveling mud.

On 05/03/2023 IE presented an EE with work restrictions ordered by a physician for shoulder pain. EE believes that the shoulder pain was caused while using an impact wrench in early April. IE did not report the injury to the employer until 05/03/2023.

Employee was climbing out of a chute with a lit torch in left hand when they swiped their right hand on the torch. Employee sustained a third degree burn to the right hand.

While driving a manlift between plants the operator hit a rut and the manlift tire dipped, jarring the operator in the basket. The operator hit EE's hand on the handrail creating a non-displaced fracture of a bone in EE's hand.

Operator had EE's hand pinched between a head pully and a bracket, resulting in a cut that required stitches to close the wound.

Employee was being trained in a new task, changing out separator fan blades, when their right foot slipped and caught a fan blade resulting in a strain/sprain.

EE was unloading Sulphur and got the supplied breathable airline caught on the heel of their boot when stepping backward. This caused EE to trip and fall on their backside. While trying to brace from the fall with hands EE hairline fractured a bone in left wrist.

Employee was rough loaded by electric shovel.

EE was coming out of the man-elevator and stepped off of the step. They rolled their right ankle. Employee fell to the ground. Ankle began swelling. Employee was taken to the clinic and placed on restrictions.

Employee was on the 8 wash screen changing poly decks. Employee was using a screwdriver to pry pin out of the poly screen. Screwdriver broke and the employee fell forward into the broken screwdriver puncturing their left wrist.

An employee was operating a CAT 777 Haul Unit in the L2 pit, when a boulder rolled out of the muck pile and struck the dovetail of the 777. The employee was staged while being loaded by a 992 when the incident occurred. As a result of the incident, the employee sustained soft tissue injuries to the neck and lower back.

Employee placed the loader bucket pin on top of the bucket. Employee then bent down to guide the operator to properly line up the bucket/boom arm hole. The pin rolled off the bucket and hit the employee's head.

Mechanic (M1) was tasked with working with an automated welding machine. During welding activities M1 began experiencing pain/itching in both of their eyes. M1 was diagnosed with flash burns and received prescription medication for both eyes.

On May 31st, 2023, an employee was working to remove the crowd brake on 935 electric shovel to replace broken bolts. When the final bolt was removed, the stored energy in the crowd brake caused the brake to rotate. When the brake rotated, the employees arm came into contact with it. This contact caused lacerations to the employees' arm, which resulted in stitches.

Employee was using 2 wrenches to tighten a bolt on a strip vessel. One of the wrenches slipped of the bolt and struck the employee on the upper lip. This caused a 1/4"" laceration.

Lifting a check valve shell with the over head crane and was applying tension to the choker with EE's left hand when the choker shifted and pinched EE's left middle finger resulting in a laceration requiring 2 stitches.

Employee was loading ANFO from an overhead bin into a Powder Monkey truck. EE pushed the lever to open the bin and when EE released it, it swung back hitting EE in the mouth. Employee sustained a laceration on the lip and a chip in an upper right tooth.

The employee was pulling the cat pads back together on the #7 Miner Serial #JM 6446A when the chain ratchet slipped off striking the employee on the left pinky finger. The incident resulted in a fractured left pinky finger. the entry was approximately 6 feet High and damp.

Miner was shooting cardox in kiln, inserted the drill bit into the port and when the air valve was turned on hot clinker dust blew back into the miner's face and arms. The miner received first, and second degree burns to face and arms. Miner was treated at ER Clinic and returned to work the same day.

K order in place investigation on going. 6/2/23 - K Order still in place pending equipment retrieval plan approval. Preliminary investigation showing mechanical failure. Waiting for equipment to be able to make final determination.

The core innertube hung up in the top of the drill rods on a +25 up-hole. The Helper placed a hand on the innertube to shake it and when it came free, their right hand fingers were crushed between the top of the innertube latch head and the sheave wheel bracket on the rig frame.

Employee stated employee was cutting rubber pipe discharge line when employee's right hand slipped out of the pipe cutting left hand. Employee received 5 stitches in left hand.

EE was removing an electrical jumper cable from the rib and felt a ""pop"" in EE's stomach. EE was diagnosed with a small hernia. EE continued mining at full capacity. EE elected to have a procedure to fix the small hernia on 7/3/2023. EE returned to work after procedure on EE's next scheduled shift of 7/8/2023.

While removing a filter on a haul truck with a strap wrench, mechanics hand slipped off wrench and the handle hit EE in the mouth resulting in a chipped tooth.

Employee alleges EE was removing a guard from a water pump the guard behind EE fell on EE's left foot. EE received a fracture to EE's foot.

Employee was removing a large rock from the bin feeder and EE's hand was smashed, causing injury.

Employee was cleaning rust/debris from floor in the engine room. The generator kicked on and exhaust air stirred up dust, causing irritation to left eye. Employee sought medical attention on 5-27-23 after irritation persisted overnight; no foreign objects were found to be in the eye: however, the DR prescribed an Rx antibiotic eye drop as a precautionary measure.

Employee's left index finger was struck with a piece of 1"" by 1"" stock metal while employee was holding back a chop saw guard that was in the process of getting it's cutting wheel replaced. The index finger required 11 stitches.

While disconnecting hose from the water pump, the camlock flew back and hit the miner in the chin. Needed 4 stitches.

Employee was walking towards the mantrip when EE tripped and fell on the mine floor, while catching self EE received a cut to EE's left thumb that required 5 stitches.

Injured was driving an insert in the drum of the continuous miner when a small object flew and hit injured in the upper part of the right cheek. Injured had small sliver of metal removed from the cheek and received sutures.

Employee pinched finger in swing gate requiring sutures.

Employee captured HF acid in a bottle, placing it in their pocket and the bottle ruptured as EE walked back to plant burning EE's skin with chemicals.

Subject was changing a 2 inch pipe on the fuel truck. The pipe slipped off the truck and hit the subject on the right side of the face.

Fixing a 988B Cat loader, putting a new bucket pin in and it slipped out and smashed left thumb with hammer. Thumb needed sutures.

Employee was working near the section conveyor tailpiece, when a rock came through the tail piece knocking the guard loose striking the employee causing a laceration around left eye. Resulted in 16 sutures.

Employee was sitting in their vehicle during lunch while a storm was passing. Once given the ""ALL CLEAR"" to resume work activities, employee began to gather materials to return to work. A bolt of lightning struck very close to them. Employee fell to the ground. Foreman ran to employee's aid.

Employee was lifting a mill liner when they cut their hand.

Employee was lying down underneath chute of stationary crusher attempting to dislodge limestone that was stuck. rock was dislodge and additional rock that employee did not see also fell striking EE in the right portion of EE's forehead

Employee was attempting to install a new wheel unit on a shuttle car and as employee was attempting to bolt it up the wheel unit came loose and fell over on employee's right arm.

Employee was carrying a spray bar and walking in #1 shaft return tunnel. Employee hit EE's head on the top of the tunnel, knocking EE's hard hat off. EE bent down to get EE's hard hat and hit self in the mouth with the spray bar chipping EE's tooth. Employee scheduled to have repair done on May 30, EE reported to Safety Dept. that the repair was complete 6/12/23.

Miner finished welding under the primary jaw crusher while wearing a welding/grinding hood. They switched to a face shield and used a wire wheel to clean the welds. They removed the shield, put on their safety glasses, and began moving the tools out from under the jaw crusher. Wind moved some of the dust as they climbed out. They then felt something in their left eye.

Employee was loading a broken heel plate from a loader into the back of a service truck. EE cut left pinky finger on the plate as EE tossed it into the bed of the truck. EE received 8 sutures and was released to full duty.

While switching plant over employee needed to use a come along to flip a gate, once the come along was grabbed the chain was knotted up. The employee hung the come along up to straighten the chain and get some slack in it, when EE pulled the slack the hook came free and struck employee in the mouth breaking one front tooth in half.

worker was grinding/torching when debris got in EE's eye

A crusher hand with approximately 3 months of experience broke two fingers while performing routine maintenance on crusher apron. EE was able to return to work the following day

Employee was tying rebar mat on the ground and tripped. Employee got laceration to right arm from falling on end of rebar.

EE was lifting a 30 lb. bucket from ground level to the back of EE's service truck, when EE felt a sudden pain in left, lower back.

Employee had hand on canopy of underground forklift while operating. Canopy contacted mine rib pinching left hand. Resulted in a laceration requiring 13 sutures.

EE is a splitter. EE was stacking slate and felt a pop in the lower back.

EE was working Saturday when leaving the maintenance trailer, EE was locking the door when a bee stung EE just below the left eye, and two more bees stung EE on either side of the bridge of EE's nose. EE ran to the loading dock area, talked to another EE, clocked out at the loading dock, then made EE's way to EE's truck. By the time EE got home EE's left eye was swollen.

On 5/11/23, operator was using a dolly to move ten-50lb bags of material. Operator pulled dolly back, lost their footing and caused the momentum of the material to push the operator down and they fell on their left knee. The operator was treated with first aid for knee abrasion. Operator did not want to see a doctor when injury occurred. Operator requested to see a doctor 5/19/23.

Employee was using a self-retracting razor knife to replace a sweep on the jaw crusher discharge belt. While cutting the sweep, employee was not wearing cutting gloves and overrode self-retracting action of knife. While cutting, left hand slipped under blade, cutting through regular glove and into left pinky finger - EE was taken to urgent care and received 4 stitches.

EE was helping to change out dust collector cartridges when dust or fiber from dust cartridge entered into the top of employees safety glasses causing a scratch on employee's eyeball

Employee was pulling HWM cable with J hook and felt a pop in right elbow.

Our employee was cleaning overflow material out of a cone crusher when they pinched their right ring finger between two rocks, causing a laceration and damaging the fingernail. They sought medical treatment and were also given a finger splint.

While lifting a section of walkway grating into position the worker's gloved right pinky finger was pinched between the grating and the frame it sits on causing a pinched finger with a minor cut.

Employee was hanging belt structure with chain. As they wrapped the excess chain around the bedrail, the end of the chain hit them in the mouth, chipping a tooth.

The employee strained their back in February while lifting a tire. The employee received no medical treatment for the injury. While receiving a bone scan from a non-occ issue it was discovered that the employee had a fracture to a vertebra. The employee believes this is from the strain they suffered in February.

An employee was in the process of breaking down cardboard boxes for recycling. Employee slid a bare hand across the box flap contacting a 3"" staple that entered the bottom and exited the top of the left-hand index finger. Staple was removed by the PA, and the employee returned to work without any restrictions.

EE was lifting an empty pallet that was caked with product residue. The pallet was heavier than originally thought. EE dropped the pallet hitting their fingers.

Employee was walking around the lube truck at 114 break F-Mains when employee slipped and fell. When falling, something cut employee's leg above right knee, causing a laceration. Employee received 4 stitches.

The employees drill steels became cross-threaded and popped EE in the left hand causing a laceration in the webbing between EE's index finger and thumb. (6) sutures were required to close the wound.

Employee had a gloved hand on a lift chain when employee gave the command to raise the load. Employee's glove was pinched in the chain pulling employee's finger tip in the pulley before employee pulled hand out of the glove. Resulting in a fractured finger tip.

Employee looked up and dust from the rim of EE's face shield fell into EE's left eye. EE was wearing safety glasses under the face shield. flushed eye and finished shift. Went to doctor next morning and was prescribed eye drops, nothing found in EE's eye.

tire bar slipped out of tire and cut their lip 10 stitches

A miner was performing a routine crusher liner inspection, when a steel bar used in opening and closing the access door struck the employee in the right forearm resulting in a contusion. This incident became reportable on 7/25/2023 when physical therapy was prescribed.

MMG was placing wheel chock stand from bed of truck around wheels of parked vehicle. Wheel chock handle laying down in bed of truck, rotated upward unexpectedly striking MMG on right side of safety glasses. Resulted in a cut (stitches) and bruise to right upper cheek of face.

Employee was weed-eating the bank behind the office building when they slipped on the bank. The employee fell backwards landing on left shoulder and stove it up.

Miner was standing on a 6 foot step ladder when the miner tried to step down the miner's foot got caught and the miner fell head first with head and shoulder hitting the ground first. ***Miner had personal neck surgery previously. No medical would see them. We took next available with previous surgeon. Miner was immediately released to full duty.***

The worker was struck in the shoulder with a 150-200lb bearing that was suspended from a nylon strap that was holding the load. The strap broke and the load fell & bounced approx. 18-20 ft and traveled approx. 11-15 ft before striking worker. The work above was being conducted by another contractor PAR

Injured was washing a coal hauler when the water splashed back against the top causing debris to get in the right eye. Injured had eye flushed and was giving prescription eye drops.

Employee was doing an inspection of the shovel dig face when they slid down the embankment.

Changing hoses in mill to set up running a different product. While trying to maneuver one of the hoses, it slipped out of the employee's hand and hit below employee's left eye and cut the left check bone. Employee left work for a short period of time to receive stitches, and returned for the remainder of the shift.

Employee was cutting expansion metal with a diamond wheel cutting disc on EE's grinder. While cutting the metal the grinder kicked back cutting EE's right hand on the thumb.

EE WALKING AROUND POWDER TRUCK AND HIT RIGHT KNEE ON A PIECE OF ANGLE IRON RESULTING IN LACERATION REQUIRING SUTURES. EE WAS RELEASED BACK TO FULL DUTY FOLLOWING SUTURES.

While walking beside the loading machine, the employees hand struck a Pie pan rib support causing a laceration on left index finger knuckle. Employee received 4 sutures to close laceration. Employee returned as scheduled, no lost time.

Employee was using a grinder and cutting disc to cut a u bolt on a trailer. While cutting the bolt the disc broke striking employee in the wrist and chest area causing lacerations one to the wrist area and one to chest area.

Socket slipped off the allen head bolt and allowed forearm to hit the machining rail causing a cut. The cut was wrapped and employee was taken in to the doctor for 11 stitches. Dr report states: laceration without foreign bodies. Employee was medically released without restrictions, and returned to work.

Employee was grinding a bolt overhead when debris fell down into right eye.

Employee was using a wrench hand tool to loosen up a nut threaded on a bolt. Employee stated when the nut broke free the employee's momentum took their hand forward resulting in left pinky finger being caught between the wrench and guarding. Employee's left pinky finger was cut and broken.

Employee was setting timber when they bent over to pick up wedges a timber fell striking them on the head above the left ear.

An employee was working on a roof replacing metal roofing. EE got to the end of the SRL and it pulled EE back. Causing EE to trip. EE tried to catch self with left arm.

While prepping a conveyor belt Splice employee cut left thumb with a skiving tool. 9 stitches.

Employee was carrying a receiver plate and tripped on a mixed gas hose and fell with the plate and the plate contacted the employee on the left eye brow. This caused a laceration over the left eye brow needing stitches.

While accessing operator's deck of a scoop, employee came in contact with a weld burr lacerating left arm resulting in five (5) sutures.

The employee swung a hammer, missed the target, and the momentum carried the swing beyond the target to the drill mast. The handle hit the mast cutting/pinching the employee's right index finger. The employee received three stitches at the emergency room, and the doctor cleared the employee for full duty.

The Driller assistant tightened a tube assembly. When downward pressure was placed on the hand tool the tube head assembly pulled up and stuck the employee in the upper lip.

Employee states a gust of wind blew a metal shaving from welding into their eye.

Employee was using the crane on a service truck to lift a CAT D11 final drive in the shop. EE was turning the hub with the crane and using a chain to guide the hub. The hub turned quickly and pulled the guide chain and in turn pulled EE's shoulder. EE experienced pain in right shoulder. Did not receive any medical treatment until 6/23/2023 when EE started therapy.

Mechanic removed cutting edge from loader bucket. Following removal and placement of old cutting edge inside bucket, they pushed the cutting edge out of the way. In doing so, the edge lacerated glove through to left index fingertip. 4 stitches required.

Miner was mucking the face after blast and hit a toe in the sill which caused the mucker to stop abruptly, miner hit their head in the cab.

On 5/12/23 after pulling open a sand box truck lid an employee noticed an abdominal protrusion. On 7/20/23 the employee was diagnosed with an abdominal hernia. Surgery to repair the hernia is scheduled for 8/3/23.

A Laron employee on upper end of the #2 Apron Feeder repositioned their body, laid down a sleever bar employee was using on the structure when it began to fall approximately 25 feet toward a WIRC employee striking the ear on the right side of the head lacerating the ear.

While changing a fuel transfer pump, technician went to clean off oil pump hardline with Brake Kleen, sprayed above EE's head but the nozzle was facing EE instead of away. Brake Kleen hit EE in the eyebrow and ran down under EE's safety glasses into EE's eye. Immediately flushed eye out at the eye wash station, then flushed again with saline solution.

I.E. was driving roofbolter from cab while pulling up cable slack to last open. As they were backing around corner, they leaned back to see the light that was flagging them. As the I.E. leaned back they came in contact with the coal rib pinching the back side of their upper right arm. This caused two lacerations that required stitches. I.E. has not missed work due to the injury.

Employee was using a knife to cut a zip-tie off a hose hanger supporting hydraulic hoses. EE had left hand on the hose holding it in place while they cut the zip-tie away. As they cut the zip-tie away, the knife contacted left hand cutting the top of middle finger. Received 3 stitches to close the cut. Accident caused by employee's failure to use a knife in a safe manner.

Miner stepped on board that was hidden under muck. This board had a nail in it and the miner stepped on this nail with EE's left foot causing a puncture wound.

Employee was closing the dividing wall in a conference room. They attempted to push it an additional 4 inches to the rubber bumper. At this point the employee kneeled to push against the wall. They then felt a sharp sting and pain in their right foot after pushing the wall. Employee must wear a Cam boot as result of incident.

Feed Operator was walking across the concrete feed pad and stepped on a rock, rolling left ankle.

Changing a rear idler on the dozer, idler was stuck in place so employee tried striking it with a sledgehammer. Piece of metal flew off and embedded into the right side of employees chest. Foreign body removed and wound was sutured.

Sawing, went to pull on a block and heard something pop. Went to doctor and they suspect a muscle tear/pull.

Employee was operating water truck to reduce dust is high wind conditions. Wind blew dust debris into employees left eye.

Right hand was caught between a chain and a block, causing a pinched right index finger

An EE was injured when they stepped back on the Roof Bolter Operator Platform, and missed footing. They fell between the platform and the TRS Scissor-lift frame. They struck the left side of their face on the frame, causing a laceration to the left side of their face. It required 3-stiches to close the laceration.

Miner was removing a snap ring from a crusher eccentric. It came apart with more force then expected and slapped the miner's finger against the wall of the eccentric. Causing laceration to the left pinky finger.

After using a metal hammer to adjust a gate, employee tossed the hammer. Unbeknownst to the employee, there was a metal ""burr"" on the hammer that cut their right index finger. Employee was taken to medical clinic for treatment. The employee's finger was treated with 10 stitches, and employee was released to return to work.

Using a hammer to put a miner chain back together when a piece of metal came off the chain and hit IE in the forehead. IE received 1 suture.

Employee was standing at regrind area when dust/debris was blown everywhere from the wind causing the dust/debris to enter employee's right eye. On 5/17/23 employee was prescribed oral antibiotics and eye drops. Employee has been referred to a specialist.

Miner strained right knee while stepping down from a ladder.

As the employee was attempting to cut pump packing, the utility knife slipped and punctured a leg. Based on the depth of the puncture wound, the employee was transported to the local health care facility for medical treatment. The employee received three stitches to close the wound. The employee was released for full duty without restrictions.

Employee hit shoulder on serge tunnel wall and caused a contusion. Was seen by doctor but release to full duty. On 6-27-2023 employee was seen by doctor and given a shot. Making it a recordable released to full duty again.

EE was hoisting a sun gear off a flat car using a chain hoist. The weight of the chain hoist caused the rope to slip. The EE's pinky was caught in the rope causing a laceration and fracture.

Employee was walking with a short flathead shovel into the tool room at the process plant when EE tripped over a cushion mat on the floor, while trying to catch themselves the shovel handle made contact to the front teeth causing the left one to chip in half.

The employee was removing a fire suppression canister for a scoop charger out of a scoop bucket, it caught EE's right pinky finger between the canister and the charger causing an abrasion

Employee was removing cutting edge from 966 loader. While removing the carriage bolts from the cutting edge, the bolt dropped down and started to turn. The employee pushed the bolt back up with right index finger, the bolt turned again cutting EE's finger requiring 7 stitches.

I.E., stated EE was replacing a collapsed tube when I.E., head was pulled up to the tube due to the suction. I. E. went to the hospital after shift and received stitches.

Deckhand was moving a cable when a burr went through their leather glove and punctured left pinky. The burr had to be removed at the hospital. Antibiotic was prescribed and tetanus shot given. Returned back to work. No other treatment given.

-Miner Working in Line of Fire. -Overconfident in the task. -Did not follow company standards; wrong gloves for the job, did not do a 60's to think before doing the task. Employee was then taken to local ER where employee received 4 stitches in right index finger.

Employee was assisting with unrolling conveyor belt when at the end of the roll the chain holding it came loose allowing the bar to fall and strike EE's left hand resulting in a laceration. The injury resulted in 13 sutures.

Employee was walking under a piggy back conveyor and struck the lower support. The employee was wearing a hardhat.

While Employee was cleaning up rock in the pit where the rock trucks where backing in, unaware that Employee was on an unstable rock under the excavator Employee exstended the arm out to move a rock and the excavator tipped over on its right side. The operator recevied two cuts to head and cuts to right hand. Employee was wearing seat belt.

Team lift of a purlin being performed when wind gust caused one employee to lose control of the purlin, which rolled onto right wrist lacerating it. Leather gloves & long sleeves were being worn; proper team lift techniques were followed. Bleeding was slowed and employee transported to ER for treatment - 10 stitches and a tetanus shot. Released with no work restrictions.

Employee was assisting with the installation of a conveyor belt and cut a finger.

Employee was using a knife to cut mesh. Employee was cutting toward themselves and slipped, causing the knife to penetrate their right thigh leading to a laceration. Employee received 2 stitches to close the wound.

Employee swung a hammer to close a feed gate at an odd angle to prevent a screw to overload. Employee missed the feed gate and smacked left index finger.

Employee cut two fingers while cutting a zip tie off of a hydraulic motor on a drill with a utility knife. Employee received 14 sutures in a medical facility.

EE was lifting up a grinder and touched the trigger on it and it cut EE's forearm.

REPORTABLE DUE TO FRACTURE/SUTURES: Employee was using a pry bar to remove/change a stuck top center roller on 2 West belt when the bar slipped and EE's right-hand pinky finger was caught between the bar and the whole frame crushing the tip of the finger causing a fracture and a laceration which required sutures. (Reported back to work for EE's next shift on 5/8/23)

***Due to positive drug test requesting this incident be non-chargeable.*** *** Unknown last 4 of SSN - temp agency and temp worker refuse to provide.*** A temporary lab tech was using masonry saw to cut core samples when thumb came in contact with blade and cut EE's thumb requiring stitches. Temp was terminated due to positive drug results.

Employee was working in the chemical lab and was washing a 800 ml beaker and it slipped out of their hands and hit the edge of the sink. The glass from the beaker cut their left forearm causing a laceration. Employee required sutures to close the wound.

Injured Person stated while picking up tools for shift end, IP felt a nick on hand and during the next shift the right hand swelled. IP reported the incident to supervisor on 5-6-2023 at 7:00 am and went to the local ER for further observation. IP was treated for a minor lime irritation to an open wound. IP was treated and released back to work without any restrictions.

WHILE UNLOADING TIMBERS- EMPLOYEE INJURED RIGHT KNEE

THIS INCIDENT BECAME A RECORDABLE ON 6/8/23 WHEN THE EMPLOYEE HAD A FOREIGN BODY REMOVED FROM THE RIGHT EAR CANAL. While welding a spacer plate, a small piece of slag entered the right ear. The employee thought the piece of slag exited the ear. It was determined that it hadn't.

Employee was cutting lagging material to go around the main crusher. EE was using a box knife to open a hole for a bolt when the knife slipped cutting EE's right thumb.

While recovering room set up, individuals right hand got caught on a cable support hanger resulting in a laceration needing 9 sutures.

Employee was on EE's knees shoveling. When EE finished, EE put EE's hand down to help self up and contacted a sharp rock with EE's hand, resulting a cut that needed stitches.

An employee was injured while attempting to make repairs to an underground slurry line. As the last of four bolts was being removed stored pressure sprayed lime slurry on the EE injuring right eye.

Employee claims they had some issues with their back about three weeks ago but reported it today.

the employee was walking in the shop when tripped and stuck Left hand out to catch themselves on a box. Their hand caught a screw on the box and cut their hand. resulting in 7 stitches to the left hand.

Miner was on the plant clearing the roll box while the plant was running when a rock that was estimated 6 inches fell roughly 4-6 feet from the top deck and struck the miner. Miner was wearing a hard hat at the time and there was a witness that saw the incident occur. There was no open injuries or bruising.

While manipulating a shackle on the gearbox, an Enscicon employee's finger was pinched between the shackle and the gearbox. This incident was initially reported as first aid, then escalated to a medical treatment injury after a dr visit on 5/12/2023.

Employee was assisting another employee hold packing around shaft and received a laceration requiring 5 stitches.

Employee was attempting to unhook the winch hook while offloading supplies. The operator accidentally winched in instead of out causing the employee's finger to get caught between the winch hook and scoop bucket causing injury.

EE states that EE was in the process of removing the drill steel. As EE reached to pull the drill steel from the rib a piece of kettle bottom became dislodged striking the EE's hand against the drill pot causing a laceration and fracture.

EE was cutting the CM Hose with a utility knife and hand slipped, slicing the left-hand ring finger. EE called the Triage Hotline number for support on dealing with the injury. EE cleaned the cut and applied Neosporin and a band-aid to the affected finger. EE was seen at Local Urgent Care and was informed that the cut would require stiches.

Two employees were replacing guarding on the side of a conveyor. The guard slipped and pinched the left index finger between the guard and frame work, resulting in need of stitches.

EE was working on some parts and the door to the area EE was working in started to shut and EE stuck hand in to catch it.

Operator when entering a loader, pinched finger between the door and the door frame. Operator refused treatment on first day. Went in for treatment on 5/2/2023. Diagnosed with fracture of the fingertip.

While using a hammer to hit a punch, the employee missed striking left thumb. This resulted in 9 stitches and a fractured bone at the end of the thumb.

EE was preparing to work on the panel of the right-side dump S/C. The rub rail was bent against the panel lid. EE jerked the panel lid and it dropped striking EE on the left middle finger causing a fracture and laceration.

Employee was sawing and went to lift a block off the rolls and pulled a muscle in their rib area.

Contractor was lowering spacer blocks behind a bearing on the 6-3 nose roller. The spacer block slipped and contractor's finger slipped into 1 of the bolt holes on the spacer block causing a laceration to the left ring finger requiring sutures.

When employee was cleaning screen deck with supervisor and another employee, they exerted themselves and became dehydrated.

The employee was tasked with performing maint on a compressor. Upon completion of routine maint, the employee replaced the door. The employee reported they felt pain in bicep. The task has been repeatedly completed by the employee weekly throughout EE's career. EE was evaluated by the medic and transported to the hospital. Surgery was conducted to repair the tendon

Employee received a second, conforming, B-read of EE's chest x-ray indicating a ""1/1"" degree of profusion in small opacities. Note: One of the B-readers noted that employee is a smoker.

Routine work as a mobile equip mechanic.

Exposure to Coal Mine Dust

Miner has been diagnosed with pnuemoconiosis.

Ee reports pain that began in June or July of 2022. No specific incident. Off work effective 3/30/23, went on STD. Following MRI, diagnosed with herniated disc T9-10. Specialist told ee that this appears to be caused by cumulative trauma at work. Filed WC claim 5/19, WC claim accepted 5/25/23.

During a review of audiograms by our audiologist, it was determined that our employee sustained a reportable threshold shift in the left ear on May 17, 2023. Employee is being retrained in accordance with section 62.180 of the standard.

Employee was awarded 15% permanent partial disability for occupational pneumoconiosis.

The contractor was coming up the stairs when the contractor felt dizzy and passed out.

EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME / CUBITAL TUNNEL SYNDROME ON 5/9/2023. TREATMENT AND/OR SURGERY IS PENDING AT THIS TIME.

Miner Showed a Standard Threshold Shift in annual audiogram results.
